Aims. This study aimed to determine whether lateral femoral wall thickness (LWT) < 20.5 mm was associated with increased revision risk of intertrochanteric fracture (ITF) of the hip following sliding hip screw (SHS) fixation when the medial calcar was intact. Additionally, the study assessed the association between LWT and patient mortality. Methods. This retrospective study included ITF patients aged 50 years and over treated with SHS fixation between 2019 and 2021 at a major trauma centre. Demographic information, fracture type, delirium status, American Society of Anesthesiologists grade, and length of stay were collected. LWT and tip apex distance were measured. Revision surgery and mortality were recorded at a mean follow-up of 19.5 months (1.6 to 48). Cox regression was performed to evaluate independent risk factors associated with revision surgery and mortality. Results. The cohort consisted of 890 patients with a mean age of 82 years (SD 10.2). Mean LWT was 27.0 mm (SD 8.6), and there were 213 patients (23.9%) with LWT < 20.5 mm. Overall, 20 patients (2.2%) underwent a revision surgery following SHS fixation. Adjusting for covariates, LWT < 20.5 mm was not independently associated with an increased revision or mortality risk. However, factors that were significantly more prevalent in LWT < 20.5 mm group, which included residence in care home (hazard ratio (HR) 1.84; p < 0.001) or hospital (HR 1.65; p = 0.005), and delirium (HR 1.32; p = 0.026), were independently associated with an increased mortality risk. The only independent factor associated with increased risk of revision was older age (HR 1.07; p = 0.030). Conclusion. LWT was not associated with risk of revision surgery in patients with an ITF fixed with a SHS when the calcar was intact, after adjusting for the independent effect of age. Although LWT < 20.5 mm was not an independent risk factor for mortality, patients with LWT < 20.5 mm were more likely to be from care home or hospital and have delirium on admission, which were associated with a higher mortality rate. Aims. The aim of this study was to investigate the association between the type of operation used to treat a trochanteric fracture of the hip and 30-day mortality. Patients and Methods. Data on 82 990 patients from the National Hip Fracture Database were analyzed using generalized linear models with incremental case-mix adjustment for patient, non-surgical and surgical characteristics, and socioeconomic factors. Results. The use of short and long intramedullary nails was associated with an increase in 30-day mortality (adjusted odds ratio (OR) 1.125, 95% confidence interval (CI) 1.040 to 1.218; p = 0.004) compared with the use of sliding hip screws (12.5% increase). If this were causative, it would represent 98 excess deaths over the four-year period of the study and one excess death would be caused by treating 112 patients with an intramedullary nail rather than a sliding hip screw. Conclusion. There is a 12.5% increase in the risk of 30-day mortality associated with the use of an intramedullary nail compared with a sliding hip screw in the treatment of a trochanteric fractures of the hip

Aims. The aim of this study was to investigate if there are differences in outcome between sliding hip screws (SHSs) and intramedullary nails (IMNs) with regard to fracture stability. Methods. We assessed data from 17,341 patients with trochanteric or subtrochanteric fractures treated with SHS or IMN in the Norwegian Hip Fracture Register from 2013 to 2019. Primary outcome measures were reoperations for stable fractures (AO Foundation/Orthopaedic Trauma Association (AO/OTA) type A1) and unstable fractures (AO/OTA type A2, A3, and subtrochanteric fractures). Secondary outcome measures were reoperations for A2, A3, and subtrochanteric fractures individually, one-year mortality, quality of life (EuroQol five-dimension three-level index score), pain (visual analogue scale (VAS)), and satisfaction (VAS) for stable and unstable fractures. Hazard rate ratios (HRRs) for reoperation were calculated using Cox regression analysis with adjustments for age, sex, and American Society of Anesthesiologists score. Results. Reoperation rate was lower after surgery with IMN for unstable fractures one year (HRR 0.82, 95% confidence interval (CI) 0.70 to 0.97; p = 0.022) and three years postoperatively (HRR 0.86, 95% CI 0.74 to 0.99; p = 0.036), compared with SHS. For individual fracture types, no clinically significant differences were found. Lower one-year mortality was found for IMN compared with SHS for stable fractures (HRR 0.87; 95% CI 0.78 to 0.96; p = 0.007), and unstable fractures (HRR 0.91, 95% CI 0.84 to 0.98; p = 0.014). Conclusion. This national register-based study indicates a lower reoperation rate for IMN than SHS for unstable trochanteric and subtrochanteric fractures, but not for stable fractures or individual fracture types. The choice of implant may not be decisive to the outcome of treatment for stable trochanteric fractures in terms of reoperation rate. We conducted a randomised controlled trial to compare external fixation of trochanteric fractures of the femur with the more costly option of the sliding hip screw. Patients in both groups were matched for age (mean 67 years, 50 to 100) and gender. We excluded all pathological fractures, patients presenting at more than one week, fractures with subtrochanteric extension or reverse obliquity, multiple fractures or any bone and joint disease interfering with rehabilitation. The interval between injury and operation, the duration of surgery, the amount of blood loss, the length of hospital stay and the cost of treatment were all significantly higher in the sliding hip screw group (p < 0.05). The time to union, range of movement, mean Harris hip scores and Western Ontario and McMaster University knee scores were comparable at six months. The number of patients showing shortening or malrotation was too small to show a significant difference between the groups. Pin-track infection occurred in 18 patients (60%) treated with external fixation, whereas there was a single case of wound infection (3.3%) in the sliding hip screw group

Aims. This study evaluated variation in the surgical treatment of stable (A1) and unstable (A2) trochanteric hip fractures among an international group of orthopaedic surgeons, and determined the influence of patient, fracture, and surgeon characteristics on choice of implant (intramedullary nailing (IMN) versus sliding hip screw (SHS)). Methods. A total of 128 orthopaedic surgeons in the Science of Variation Group evaluated radiographs of 30 patients with Type A1 and A2 trochanteric hip fractures and indicated their preferred treatment: IMN or SHS. The management of Type A3 (reverse obliquity) trochanteric fractures was not evaluated. Agreement between surgeons was calculated using multirater kappa. Multivariate logistic regression models were used to assess whether patient, fracture, and surgeon characteristics were independently associated with choice of implant. Results. The overall agreement between surgeons on implant choice was fair (kappa = 0.27 (95% confidence interval (CI) 0.25 to 0.28)). Factors associated with preference for IMN included USA compared to Europe or the UK (Europe odds ratio (OR) 0.56 (95% CI 0.47 to 0.67); UK OR 0.16 (95% CI 0.12 to 0.22); p < 0.001); exposure to IMN only during training compared to surgeons that were exposed to both (only IMN during training OR 2.6 (95% CI 2.0 to 3.4); p < 0.001); and A2 compared to A1 fractures (Type A2 OR 10 (95% CI 8.4 to 12); p < 0.001). Conclusion. In an international cohort of orthopaedic surgeons, there was a large variation in implant preference for patients with A1 and A2 trochanteric fractures. This is due to surgeon bias (country of practice and aspects of training). The observation that surgeons favoured the more expensive implant (IMN) in the absence of convincing evidence of its superiority suggests that surgeon de-biasing strategies may be a useful focus for optimizing patient outcomes and promoting value-based healthcare. In a prospective, randomised study we have compared the pertrochanteric external fixator (PF) with the sliding hip screw (SHS) in 100 consecutive patients who were allocated randomly to the two methods of treatment. Details of the patients and the patterns of fracture were similar in both groups. Follow-up was for six months. Use of the PF was associated with significantly less blood loss, a shorter operating time, reduced postoperative pain, shorter hospitalisation (p < 0.001), earlier mobilisation (p < 0.001) and a reduced rate of mechanical complications (p < 0.01). Superficial infection was significantly more common with the PF (p < 0.01), but without long-term adverse consequences. There were no differences in the healing of the fracture, mortality or final functional outcome. Our results indicate that the external fixator is an effective and safe device for treating pertrochanteric fractures and should be considered as a useful alternative to conventional fixation with the sliding hip screw

Aims. This study aimed to compare mortality in trochanteric AO/OTA A1 and A2 fractures treated with an intramedullary nail (IMN) or sliding hip screw (SHS). The primary endpoint was 30-day mortality, with secondary endpoints at 0 to 1, 2 to 7, 8 to 30, 90, and 365 days. Methods. We analyzed data from 26,393 patients with trochanteric AO/OTA A1 and A2 fractures treated with IMNs (n = 9,095) or SHSs (n = 17,298) in the Norwegian Hip Fracture Register (January 2008 to December 2020). Exclusions were made for patients aged < 60 years, pathological fractures, pre-2008 operations, contralateral hip fractures, fractures other than trochanteric A1/A2, and treatments other than IMNs or SHSs. Kaplan-Meier and Cox regression analyses adjusted for type of fracture, age, sex, cognitive impairment, American Society of Anesthesiologists (ASA) grade, and time period were conducted, along with calculations for number needed to harm (NNH). Results. In unadjusted analyses, there was no significant difference between IMN and SHS patient survival at 30 days (91.8% vs 91.1%; p = 0.083) or 90 days (85.4% vs 84.5%; p = 0.065), but higher one-year survival for IMNs (74.5% vs 73.3%; p = 0.031) compared with SHSs. After adjustments, no significant difference in 30-day mortality was found (hazard rate ratio (HRR) 0.94 (95% confidence interval (CI) 0.86 to 1.02(; p = 0.146). IMNs exhibited higher mortality at 0 to 1 days (HRR 1.63 (95% CI 1.13 to 2.34); p = 0.009) compared with SHSs, with a NNH of 556, but lower mortality at 8 to 30 days (HRR 0.89 (95% CI 0.80 to 1.00); p = 0.043). No differences were observed in mortality at 2 to 7 days (HRR 0.94 (95% CI 0.79 to 1.11); p = 0.434), 90 days (HRR 0.95 (95% CI 0.89 to 1.02); p = 0.177), or 365 days (HRR 0.97 (95% CI 0.92 to 1.02); p = 0.192). Conclusion. This study found no difference in 30-day mortality between IMNs and SHSs. However, IMNs were associated with a higher mortality at 0 to 1 days and a marginally lower mortality at 8 to 30 days compared with SHSs. Aims. Surgical treatment of hip fracture is challenging; the bone is porotic and fixation failure can be catastrophic. Novel implants are available which may yield superior clinical outcomes. This study compared the clinical effectiveness of the novel X-Bolt Hip System (XHS) with the sliding hip screw (SHS) for the treatment of fragility hip fractures. Methods. We conducted a multicentre, superiority, randomized controlled trial. Patients aged 60 years and older with a trochanteric hip fracture were recruited in ten acute UK NHS hospitals. Participants were randomly allocated to fixation of their fracture with XHS or SHS. A total of 1,128 participants were randomized with 564 participants allocated to each group. Participants and outcome assessors were blind to treatment allocation. The primary outcome was the EuroQol five-dimension five-level health status (EQ-5D-5L) utility at four months. The minimum clinically important difference in utility was pre-specified at 0.075. Secondary outcomes were EQ-5D-5L utility at 12 months, mortality, residential status, mobility, revision surgery, and radiological measures. Results. Overall, 437 and 443 participants were analyzed in the primary intention-to-treat analysis in XHS and SHS treatment groups respectively. There was a mean difference of 0.029 in adjusted utility index in favour of XHS with no evidence of a difference between treatment groups (95% confidence interval -0.013 to 0.070; p = 0.175). There was no evidence of any differences between treatment groups in any of the secondary outcomes. The pattern and overall risk of adverse events associated with both treatments was similar. Conclusion. Any difference in four-month health-related quality of life between the XHS and SHS is small and not clinically important. There was no evidence of a difference in the safety profile of the two treatments; both were associated with lower risks of revision surgery than previously reported. We studied the biomechanical behaviour of three sliding fixation devices for trochanteric femoral fractures. These were a titanium alloy sideplate and lag screw, a titanium alloy sideplate and dome plunger with cement augmentation, and a stainless-steel sideplate and lag screw. We used 18 mildly osteoporotic cadaver femora, randomly assigned to one of the three fixation groups. Four displacement and two strain gauges were fixed to each specimen, and each femur was first tested intact (control), then as a two-part fracture and then as a four-part intertrochanteric fracture. A range of physiological loads was applied to determine load-bearing, load-sharing and head displacement. The four-part-fracture specimens were subsequently tested to failure to determine maximum fixation strengths and modes of failure. The dome-plunger group failed at a load 50% higher than that of the stainless-steel lag-screw group (p < 0.05) and at a load 20% higher than that of the titanium-alloy lag-screw group (NS). All 12 lag-screw specimens failed by cut-out through the femoral head or neck, but none of the dome-plunger group showed movement within the femoral head when tested to failure. Strain-gauge analysis showed that the dome plunger produced considerably less strain in the inferior neck and calcar region than either of the lag screws. Inferior displacement of the femoral head was greatest for the dome-plunger group, and was due to sliding of the plunger. The dome plunger with cement augmentation was able to support higher loads and did not fail by cut-out through the femoral head.(ABSTRACT TRUNCATED AT 250 WORDS)

Aims. Despite limited clinical scientific backing, an additional trochanteric stabilizing plate (TSP) has been advocated when treating unstable trochanteric fractures with a sliding hip screw (SHS). We aimed to explore whether the TSP would result in less post operative fracture motion, compared to SHS alone. Methods. Overall, 31 patients with AO/OTA 31-A2 trochanteric fractures were randomized to either a SHS alone or a SHS with an additional TSP. To compare postoperative fracture motion, radiostereometric analysis (RSA) was performed before and after weightbearing, and then at four, eight, 12, 26, and 52 weeks. With the “after weightbearing” images as baseline, we calculated translations and rotations, including shortening and medialization of the femoral shaft. Results. Similar migration profiles were observed in all directions during the course of healing. At one year, eight patients in the SHS group and 12 patients in the TSP group were available for analysis, finding a clinically non-relevant, and statistically non-significant, difference in total translation of 1 mm (95% confidence interval -4.7 to 2.9) in favour of the TSP group. In line with the migration data, no significant differences in clinical outcomes were found. Conclusion. The TSP did not influence the course of healing or postoperative fracture motion compared to SHS alone. Based on our results, routine use of the TSP in AO/OTA 31-A2 trochanteric fractures cannot be recommended. The TSP has been shown, in biomechanical studies, to increase stability in sliding hip screw constructs in both unstable and intermediate stable trochanteric fractures, but the clinical evidence is limited. This study showed no advantage of the TSP in unstable (AO 31-A2) fractures in elderly patients when fracture movement was evaluated with radiostereometric analysis. Aims. The aim of this study was to examine perioperative blood transfusion practice, and associations with clinical outcomes, in a national cohort of hip fracture patients. Methods. A retrospective cohort study was undertaken using linked data from the Scottish Hip Fracture Audit and the Scottish National Blood Transfusion Service between May 2016 and December 2020. All patients aged ≥ 50 years admitted to a Scottish hospital with a hip fracture were included. Assessment of the factors independently associated with red blood cell transfusion (RBCT) during admission was performed, alongside determination of the association between RBCT and hip fracture outcomes. Results. A total of 23,266 individual patient records from 18 hospitals were included. The overall rate of blood transfusion during admission was 28.7% (n = 6,685). There was inter-hospital variation in transfusion rate, ranging from 16.6% to 37.4%. Independent perioperative factors significantly associated with RBCT included older age (90 to 94 years, odds ratio (OR) 3.04 (95% confidence interval (CI) 2.28 to 4.04); p < 0.001), intramedullary fixation (OR 7.15 (95% CI 6.50 to 7.86); p < 0.001), and sliding hip screw constructs (OR 2.34 (95% CI 2.19 to 2.50); p < 0.001). Blood transfusion during admission was significantly associated with higher rates of 30-day mortality (OR 1.35 (95% CI 1.19 to 1.53); p < 0.001) and 60-day mortality (OR 1.54 (95% CI 1.43 to 1.67); p < 0.001), as well as delays to postoperative mobilization, higher likelihood of not returning to their home, and longer length of stay. Conclusion. Blood transfusion after hip fracture was common, although practice varied nationally. RBCT is associated with adverse outcomes, which is most likely a reflection of perioperative anaemia, rather than any causal effect. Use of RBCT does not appear to reverse this effect, highlighting the importance of perioperative blood loss reduction. Cite this article: Bone Joint J 2022;104-B(11):1266–1272

Aims. Patients with A1 and A2 trochanteric hip fractures represent a substantial proportion of trauma caseload, and national guidelines recommend that sliding hip screws (SHS) should be used for these injuries. Despite this, intramedullary nails (IMNs) are routinely implanted in many hospitals, at extra cost and with unproven patient outcome benefit. We have used data from the National Hip Fracture Database (NHFD) to examine the use of SHS and IMN for A1 and A2 hip fractures at a national level, and to define the cost implications of management decisions that run counter to national guidelines. Methods. We used the NHFD to identify all operations for fixation of trochanteric fractures in England and Wales between 1 January 2021 and 31 December 2021. A uniform price band from each of three hip fracture implant manufacturers was used to set cost implications alongside variation in implant use. Results. We identified 18,156 A1 and A2 trochanteric hip fractures in 162 centres. Of these, 13,483 (74.3%) underwent SHS fixation, 2,352 (13.0%) were managed with short IMN, and 2,321 (12.8%) were managed with long IMN. Total cost of IMN added up to £1.89 million in 2021, and the clinical justification for this is unclear since rates of IMN use varied from 0% to 97% in different centres. Conclusion. Most trochanteric hip fractures are managed with SHS, in keeping with national guidelines. There is considerable variance between hospitals for implant choice, despite the lack of evidence for clinical benefit and cost-effectiveness of more expensive nailing systems. This suggests either a lack of awareness of national guidelines or a choice not to follow them. We encourage provider units to reassess their practice if outwith the national norm. Funding bodies should examine implant use closely in this population to prevent resource waste at a time of considerable health austerity. Cite this article: Bone Jt Open 2022;3(10):741–745

Fractures of the proximal femur are one of the greatest challenges facing the medical community, constituting a heavy socioeconomic burden worldwide. Controversy exists regarding the optimal treatment for patients with unstable trochanteric proximal femoral fractures. The recognised treatment alternatives are extramedullary fixation usually with a sliding hip screw and intramedullary fixation with a cephalomedullary nail. Current evidence suggests that best results and lowest complication rates occur using a sliding hip screw. Complications in these difficult fractures are relatively common regardless of type of treatment. We believe that a novel device, the X-Bolt dynamic plating system, may offer superior fixation over a sliding hip screw with lower reoperation risk and better function. We therefore propose to investigate the clinical effectiveness of the X-bolt dynamic plating system compared with standard sliding hip screw fixation within the framework of a the larger WHiTE (Warwick Hip Trauma Evaluation) Comprehensive Cohort Study. Increasing incidence of osteoporosis, obesity and an aging population have led to an increase in low energy hip fractures in the elderly. Perceived lower blood loss and lower surgical time, media coverage of minimal invasive surgery and patient expectations unsurprisingly have led to a trend towards intramedullary devices for fixation of extracapsular hip fractures. This is contrary to the Cochrane review of random controlled trials of intramedullary vs extramedullary implants which continues recommends the use of a sliding hip screw (SHS) over other devices. Furthermore, despite published literature of minimally invasive surgery (MIS) of SHS citing benefits such as reduced soft tissue trauma, smaller scar, faster recovery, reduced blood loss, reduced analgesia needs; the uptake of these approaches has been poor. We describe a novel technique one which remains minimally invasive, that not only has a simple learning curve but easily reproducible results. All patients who underwent MIS SHS fixation of extracapsular fractures were included in this study. Technique is shown in Figure 1. We collated data on all intertrochanteric hip fractures that were treated by a single surgeon series during period Jan 2014 to July 2015. Data was collected from electronic patient records and radiographs from Picture Archiving and Communication System (PACS). Surgical time, fluoroscopy time, blood loss, surgical incision length, post-operative transfusion, Tip Apex Distance (TAD) were analyzed. There were 10 patients in this study. All fractures were Orthopaedic Trauma Association (OTA) type A1 or A2. Median surgical time was 36 minutes (25–54). Mean fluoroscopy time was similar to standard incision sliding hip screw fixation. Blood loss estimation with MIS SHS can be undertaken safely and expeditiously for extracapsular hip fractures

Background. Intracapsular neck of femur fractures are one of the most common injuries seen in Orthopaedics. When the fracture is amenable to internal fixation there are 2 main treatment options, namely multiple cannulated hip screws (MCS) and 2-hole sliding hip screws (SHS). In this retrospective study we examine the outcomes associated with these two methods of internal fixation. At present there is little consensus regarding which treatment should be used. Methods. 161 patients were found to have suffered intracapsular neck of femur fracture treated with either SHS or MCS fixation over a 5 year period from April 2009 to April 2014, allowing at least 1 year follow up following injury. The patients imaging and clinical notes were then reviewed to ascertain the outcome of their treatment and any complications. Results. 93 patients were treated with a sliding hip screw compared to 68 that had been treated with cannulated screws. To ensure the fractures in each group were comparable in terms of fracture severity they were classified using gardens and pauwels score. The mean age of the group treated with SHS was 75.15 years, 7.69 years higher than those treated with MCS. The mean length of inpatient stay was 4.7 days longer for patients treated with sliding hip screws than those treated with cannulated screws, which is significantly more than would be predicted by age difference alone. Further patients were excluded for analysis of failure rate if they had not been sufficiently followed up, leaving 122 patients, 58 treated with MCS and 64 with SHS. A significantly higher (p=0.0136) proportion of patients treated with SHS (32%) suffered failure of their fixation compared to those treated with MCS (10%). The SHS group was further classified by whether or not a permanent derotation screw was employed. The use of a derotation screw provided protection against failure with a number needed to treat of 3.82, decreasing the failure rate to 14% which was not significantly more than the failure rate for MCS. Conclusions. We recommend that the first choice treatment for intracapsular neck of femur fractures amenable to internal fixation should be cannulated screws due to a lower rate of failure and shorter length of inpatient stay. If a surgeon has a strong preference for sliding hip screw we strongly recommend inserting a permanent derotation screw

Background: Reverse obliquity and transverse fractures of the proximal femur represent a distinct fracture pattern in which the mechanical forces displace the femur medially thus increasing the risk of fixation failure. There is a paucity of published literature in this area of trauma. This study constitutes the largest series of such fractures. Methods: Using the hip fracture registry at this institution 101 reverse obliquity and transverse fracture patterns were identified from 3336 consecutive hip fractures. All surviving patients were followed up for 1 year. Results: Of 100 patients treated operatively, 59 were treated with 1350 sliding hip screws (SHS), 22 were treated with 1350 sliding hip screw devices designed to resist medialization (3 sliding hip screws with trochanteric plate and 19 Medoff plates), and 19 were treated with intramedullary sliding hip screw devices (1 short Gamma nail, 9 long Gamma nails, 6 Reconstruction nails, 6 long Targon nails, 1 short Targon nail). The SHS had 4 failures (6.8%), and the intramedullary devices one failure (5.3%). Those extramedullary devices augmented to prevent medialization had higher failure rates (1 of 3 SHS with trochanteric plate and 3 of 19 Medoff plates), with combined failure rate of 15.8%. Conclusion: The 1350 SHS and the intramedullary devices had similar failure rates of 6.8% and 5.2% respectively. Those extramedullary devices designed to prevent medialization had higher failure rates (combined failure rate of 4/22 or 18%). This is similar to the high failure rate in 950 devices reported elsewhere. This suggests that extramedullary devices attempting to combat the difficult biomechanics of these fractures are unsuccessful. Better results can be obtained by using the standard 1350 SHS or with intramedullary sliding hip screw devices

Controversy exists whether to treat unstable pertrochanteric hip fractures with either intra-medullary or extra-medullary devices. A prospective randomised control trial was performed to compare the outcome of unstable pertrochanteric hip fractures stabilised with either a sliding hip screw or long Gamma Nail. The hypothesis was that there is no difference in outcome between the two modes of treatment. Over a four year period, 210 patients presenting with an unstable pertrochanteric hip fracture (AO/OTA 31 A2) were recruited into the study. Eligible patients were randomised on admission to either long Gamma Nail or sliding hip screw. Follow-up was arranged for three, six, and twelve months. Primary outcome measures were implant failure or ‘cut-out’. Secondary measures included mortality, length of hospital stay, transfusion rate, change in mobility and residence, and EuroQol outcome score. Five patients required revision surgery for implant cut-out (2.5%), of which three were long Gamma Nails and two were sliding hip screws (no significant difference). There were no incidences of implant failure or deep infection. Tip apex distance was found to correlate with implant cut-out. There was no statistically significant difference in either the EuroQol outcome scores or mortality rates between the two groups when corrected for mini mental score. There was no difference in transfusion rates, length of hospital stay, and change in mobility or residence. There was a clear cost difference between the implants. The sliding hip screw remains the gold standard in the treatment of unstable pertrochanteric fractures of the proximal femur

Hip fracture is a common serious injury in the elderly. Between 1982 and 1998 the number of hip fractures reported annually in Scotland in patients over 55 years rose from 4,000 to 5,700. The optimum method of treatment for the various fracture types remains in contention. We compare outcome measures between displaced, intracapsular fractures in patients over 70 years fixed with cannulated screws and sliding hip screw with side plate. Between 1998 and 2005 a total of 30,482 patients were reviewed by the Scottish Hip Fracture Audit (SHFA). Of these 15,823(53.3% of the total) had sustained intracapsular fractures. 13,587 of these occurred in patients aged 70 or over. Of these 2,428 had undisplaced and 11,159 displaced fractures. Chi test statistical analysis compare outcome measures in this group of displaced intracapsular fractures with respect to aspects of early failure. 534(3.9%) of patients were treated conservatively. 509 (4.7%) fractures were fixed using cannulated screws and 499 (4.6%) using a sliding hip screw. Readmission within 120 days for any cause occurred in 62 patients(14.1%) treated with cannulated screw fixation and in 63 patients(15.7%) for those treated with a sliding hip screw(P=0.509). Of these 36 patients(8.2%) in the former and 23 patients(5.7%) in the latter group were readmitted for complications related to hip fracture(P=0.033). Mortality within this period included 69 patients(13.5%) in the CS and 98(19.6%) in the SHS group. In terms of re-operation within 120 days of the original admission, 53 patients(10.6%) receiving cannulated screws compared to 24 patients(4.8%) treated with a sliding hip screw requiring further surgery(P=0.0006). The fracture was seen to displace in 12(22.6%) patients originally treated with cannulated screws compared to 6 patients(25%) treated with sliding hip screw(P=0.156). More significantly the fixation device was seen to have migrated in 24(45.3%) of the cannulated screw as compared to 7(29.2%) patients in the sliding hip screw group(P=0.002). Periprosthetic fractures were recorded in 4(7.5%) of the former and 3(12.5%) in the latter group(P=0.708). Wound infection was higher in the SHS group(2 patients) as compared to the screw fixation group(1 patient)(P=0.565). Statistical analysis demonstrates a dramatic difference exists between these 2 fixation types in terms of re-operation within 120 days of the original admission for which published literature has previously only recorded biomechanical, in vitro comparisons. Data regarding specific implant factors such as number of screws, position, configuration, starting point, thread length and use of washers in cannulated screws, and position, tapping, supplementary screw and compression screw in sliding hip screws was not recorded and may be considered to bias our results

Aims. Evaluate if treating an unstable femoral neck fracture with a locking plate and spring-loaded telescoping screw system would improve construct stability compared to gold standard treatment methods. Methods. A 31B2 Pauwels’ type III osteotomy with additional posterior wedge was cut into 30 fresh-frozen femur cadavers implanted with either: three cannulated screws in an inverted triangle configuration (CS), a sliding hip screw and anti-rotation screw (SHS), or a locking plate system with spring-loaded telescoping screws (LP). Dynamic cyclic compressive testing representative of walking with increasing weight-bearing was applied until failure was observed. Loss of fracture reduction was recorded using a high-resolution optical motion tracking system. Results. LP constructs demonstrated the highest mean values for initial stiffness and failure load. LP and SHS constructs survived on mean over 50% more cycles and to loads 450 N higher than CS. During the early stages of cyclic loading, mean varus collapse of the femoral head was 0.5° (SD 0.8°) for LP, 0.7° (SD 0.7°) for SHS, and 1.9° (SD 2.3°) for CS (p = 0.071). At 30,000 cycles (1,050 N) mean femoral neck shortening was 1.8 mm (SD 1.9) for LP, 2.0 mm (SD 0.9) for SHS, and 3.2 mm (SD 2.5) for CS (p = 0.262). Mean leg shortening at construct failure was 4.9 mm (SD 2.7) for LP, 8.9 mm (SD 3.2) for SHS, and 7.0 mm (SD 4.3) for CS (p = 0.046). Conclusion. Use of the LP system provided similar (hip screw) or better (cannulated screws) biomechanical performance as the current gold standard methods suggesting that the LP system could be a promising alternative for the treatment of unstable fractures of the femoral neck. Aims. This study explores the reported rate of surgical site infection (SSI) after hip fracture surgery in published studies concerning patients treated in the UK. Methods. Studies were included if they reported on SSI after any type of surgical treatment for hip fracture. Each study required a minimum of 30 days follow-up and 100 patients. Meta-analysis was undertaken using a random effects model. Heterogeneity was expressed using the I. 2. statistic. Risk of bias was assessed using a modified Newcastle-Ottawa Scale (NOS) system. Results. There were 20 studies reporting data from 88,615 patients. Most were retrospective cohort studies from single centres. The pooled incidence was 2.1% (95% confidence interval (CI) 1.54% to 2.62%) across ‘all types’ of hip fracture surgery. When analyzed by operation type, the SSI incidences were: hemiarthroplasty 2.87% (95% CI 1.99% to 3.75%) and sliding hip screw 1.35% (95% CI 0.78% to 1.93%). There was considerable variation in definition of infection used, as well as considerable risk of bias, particularly as few studies actively screened participants for SSI. Conclusion. Synthesis of published estimates of infection yield a rate higher than that seen in national surveillance procedures. Biases noted in all studies would trend towards an underestimate, largely due to inadequate follow-up

We aimed to use data from a randomized controlled trial (RCT) comparing the sliding hip screw vs. intramedullary nailing (IMN) for trochanteric fractures to examine complication rates between those managed with a short vs. long IMN. This is a secondary analysis using one arm of an RCT of patients ≥18 years with trochanteric fractures. We examined differences in fracture-related (femoral shaft fracture, implant failure, surgical site infection (SSI), nonunion, limb shortening, and pain) and medical (organ failure, respiratory distress, stroke, deep vein thrombosis [DVT] gastrointestinal upset, pneumonia, myocardial infarction, sepsis, or urinary tract infection) adverse events (AE), and readmission between short vs. long IMNs. We included 412 trochanteric fracture patients, 339 (82.2%) of whom received a short (170mm–200mm) nail, while 73 (17.7%) received a long (260mm–460 mm) nail. Patients in the long group were more likely to be admitted from home (vs. an institution), and have comorbidities, or more complex fracture types. Patients in the long group had higher rates of fracture-related AE (12.3%) vs. the short group (3.5%). Specifically, SSI (5.5% vs. 0.3%) and pain (2.7% vs. 0.0%) were significantly higher in the long group. Patients in the long group were also more likely to develop DVT (2.7% vs. 0.3%), and be readmitted to the hospital (28.8% vs. 20.7%). Following covariable adjustment, long nails remained associated with a higher odds of fracture-related AE (5.11, 1.96–13.33) compared to short nails. We found no association between the adjusted odds of readmission and nail length (1.00, 0.52–1.94). Our analyses revealed that trochanteric fracture patients managed with long IMN nails may have a higher odds of fracture-related AE compared to short nails. Future research is required to validate these findings with larger event rates, and further optimize IMN for trochanteric fracture patients

Introduction: Use of a variable angle sliding hip screw is an interesting therapeutic option in pathological trauma of the proximal femur. It has successfully been used in cases of loosening of previously implanted hardware and non-union. Materials and methods: In this study we present 26 cases treated consecutively in our department between 2001 and 2005. The majority of patients in this group had suffered complications in the treatment of proximal femoral fractures, including non-unions and nail cut-outs. Results: We treated 12 male patients and 14 female patients; mean age was 72 years. Mean hospital stay was 14.3 days. The patients were examined in our consulting offices and had a minimum 6 months follow-up, with controls of their evolution at 3 and 6 months and 1 year. During the study period functional and x-ray assessments were performed. No relevant alterations were found in any of the patients; all cases resolved successfully. Conclusions: The variable angle sliding hip screw has been successfully used in cases of severe complications in patients with proximal femur fractures. We have tried to summarize our 4 years’ experience in this study by presenting a series of different cases that went from failure of a previous osteosynthesis to other techniques applied to the fracture failure such as the use of autologous grafts to resolve non-unions. The variable angle sliding hip screw is a very useful and versatile tool in the hands of the orthopedic surgeon in certain cases where rescue surgery is necessary

Purpose: The results of surgical treatment of intertrochanteric hip fractures using a sliding hip screw-plate and Norian-SRS, as an adjuvant means of stabilization, are presented. Material – Methods: 103 patients (27 male, 76 female) with intertrochanteric hip fractures, were treated with a sliding hip screw. Their age ranged from 56 to93 years (av. 68,9y) . In group A (50 patients) we only used a sliding hip screw -plate, while in group B ( 53 patients) we also used Norian-SRS above the upper surface of the sliding hip screw. Results: Postoperative follow-up ranged from 5 to17 μnνϵς. Mobilization was initiated on the 2nd day with partial weight bearing. A group progressed to full weight bearing in 3 , while B group in 2 months. 8 patients in group A and 2 in group B developed varous deformity. Screw cut out developed in 5 patient of group A and none of group B. Backsliding of the screw ranged from 0 to 16 mm. (av. 4,95 mm.) in group A and from 0 to16mm. (av. 3,25mm.) in group B. Conclusion: Norian-SRS augmentation of intertrochanteric hip fractures treated with a sliding hip screw-plate, increases the stability of the osteosynthesis, permitting earlier rehabilitation and mobilization of the patient and leading to better functional results


The Sliding Hip Screw (SHS) is currently the treatment of choice for all trochanteric hip fractures. An alternative treatment is the short femoral nail. Earlier designs of these nails were associated with an increased fracture healing complication rate in comparison to the sliding hip screw. The new designs of nails (third generation nails) may however be as good as or even superior to sliding hip screw fixation. We conducted a large randomised trial to compare the Targon Proximal Femoral Nail with the Sliding Hip Screw. Patients with trochanteric hip fractures as per the AO classification (A1–A3) were randomised to either implant. All surgery was supervised by one surgeon. All patients were followed up for a minimum of one year months by a blinded observer. The mean age was 82 years, range 27 to 104 years), 20% were male. Length of surgery was slightly increased for the nail (44 versus 49 minutes, p=0.002). Fluoroscopic screening time was increased in the nail group (0.3 versus 0.6 minutes, p< 0.0001). Intra-operative complications were more common with the nailing. There was no difference in blood transfusion requirement between groups. Postoperatively there was no difference in the occurrence of medical complications or mortality. Deep wound infection requiring removal of the implant occurred in one case in the SHS group. In addition there were two cases of cut-out, three of plate detachment from the femur and one non-union in the SHS group, requiring secondary surgery. There was only one compilation in the nailed a case of cut-out which required secondary surgery. At follow-up no difference in pain scores but there was a tendency to improved mobility in the nailed group (p=0.004). These results suggest that with improved designs and surgical technique, the newer versions of short nails for proximal femoral fractures may not suffer from the complications of the earlier short intramedullary nails. Intramedullary fixation can result in a lower re-operation rate (3.5% versus 0.5%) and improved mobility in comparison to the sliding hip screw

Introduction and Aims: The combination of ipsilateral intertrochanteric and femoral shaft fractures is an uncommon pattern associated with high-energy trauma. This retrospective study used self-report measures to evaluate functional outcome of patients sustaining this fracture pattern and compared two common treatment methods. Method: Three patient-based outcome measures, the Short Form-36 (SF-36), Short Musculoskeletal Functional Assessment (SMFA), and Lower Extremity Functional Scale (LEFS) were used to evaluate the functional outcome of twenty-one patients (13 male, mean 46.7 +/− 16.5 years) treated with a reconstruction nail (n=11) or with a sliding hip screw and retrograde nail (n=10). Results: Mechanisms of injury included motor vehicle accidents (66.7%) and falls from height (14.3%). SF-36 physical and mental component scores were less than Canadian norms, with mean values of 35.9 (p=0.0001) and 43.7 (p=0.02), respectively. There was a trend towards better functional outcome in the group treated with the sliding hip screw with retrograde nail despite this group sustaining more severe injuries as measured by ISS (p=0.004), number of days in hospital (p=0.027), and number of days in ICU (p=0.009). Conclusion: Functional outcome following treatment of ipsilateral intertrochanteric and femoral shaft fractures was reduced compared to Canadian norms. Despite having sustained more severe injuries, the sliding hip screw with retrograde nail group showed a trend towards better outcome as compared to the group treated with the reconstruction nail

Purpose: The purpose of this study is to provide an additional tool to determine the stability of AO 31A2 pertrochanteric hip fractures. This study is based on the lateral hip radiograph, which has been ignored in the current debate over stability. Method: One-hundred and thirty-one patients were identified through medical records with a diagnosis of pertrochanteric hip fracture treated with sliding hip screw from 2003–2008. Thirty-nine patients had AO 31A2 hip fractures, cross-table lateral injury films, intra-operative fluoroscopy and follow-up films. Only 23 had follow-up films beyond discharge. The landmarks of interest were angulation and translation between the femoral shaft and neck on cross-table lateral injury films. The neck was defined in three ways: the anterior cortex, two key points in the anterior cortex and the neck bisector. The most consistent measure was used. Translation of the neck was measured as a percentage of the shaft diameter. Measurements were taken by two blinded researchers with different levels of experience. Film sequence was randomized. The primary outcome was shortening of the sliding hip screw greater than one centimetre. This is the exact midpoint between 0.61 centimetres, which is not associated with reduced patient mobility, and 1.34 centimetres which is associated with reduced patient mobility, as described by Muller-Farber. The hip screw was measured from its tip to the point it enters the barrel. The diameter of the hip screw was known and provided scale. The measurement from intra-operative fluoroscopy films with the leg in traction, represented zero shortening. Results: The average follow up was 190 days. Using the neck bisector to measure angulation was most consistent (95% of measurements available versus 89% and 88% with other methods). More than 30° angulation and/or 30% translation on the lateral predicted shortening greater than one centimetre with 91% specificity and 33% sensitivity. The average shortening in this group was 1.6 centimeters, which is greater than shortening associated with reduced patient mobility (1.34 centimeters). Agreement between two researchers was 91% and considered “substantial” (kappa 0.71) as per Landis and Koch criteria. Conclusion: This is a highly specific and reproducible tool to detect a subset of AO 31A2 hip fractures which acquire unwanted collapse if treated with a sliding hip screw. This information adds clarity to the debate over stability of some AO 31A2 fracture cases, at no additional cost for the surgeon and facility. The “30/30 rule” (30° angulation and 30% translation) should not be used in isolation due to low sensitivity. Other factors may affect shortening, such as the degree of comminution and the antero-posterior film should still be considered

Purpose: The management of comminuted inter trochanteric fracture are a serious and difficult problem. The proper selection of fixation device is must to avoid significant complication in the management of this common fracture. The use of contoured side plate screw attached to the sliding screw plate anchors the comminuted fragments thus gives better stability, compression, early mobility and bony union to this fracture where other implant fails. Material and method: Since 1997 to 2000, 60 comminuted inter trochanteric fracture- age of 67yrs (46–91year) were treated by close/open reduction and internal fixation with 135 dynamic hip screw plate +side screw plate. The side plate is a narrow D.C.P. (4/5 hole) which is contoured to the fl are of greater trochanter proximally and is attached to the sliding screw plate. The proximal holes of side plate hold the comminuted fragment of greater trochanter with cancellous screw above the sliding hip screw The patient were encouraged to walk on 2nd post operative day with support to start with partial weight bearing followed by weight bearing at 4 wk. Result: On an average 12-week (8–16 wk) all the Tranzo grade II/ III fracture went into union . There was lengthening of 2.5cm (2–5 cm) in 15 cases due to valgus reduction which reduced to 1.5 cm (2–3cm) at end of 6 months. Backing up of the side plate screw and sliding hip screw was seen in 40% of case (24) (mainly in poor bone stock and valgus reduction case). Conclusion: The side plate/ screw with sliding hip screw stables buttress for comminuted trochanter fragments gives compression, prevent rotation and better bony union the piece. This implant is an extended arm for holding fragment. The major trochanteric fracture fragment are held by side plate to sliding screw plate thus gives stability to the device, better bony contact thus early mobility and union. The sliding screw device with side plate is “forgiving” it allows subsequent displacement to achieve stability in comminuted fracture. The success of the implant assembly rest on the ability of slide and hold the fragment to give stability and bony union. This method gives an option of open reduction and bone grafting

A new and very unstable intertrochanteric fracture complex is described. The intertrochanteric fracture with extension into the femoral neck is rare but results in an extremely high failure rate because of its instability. A ten-year retrospectic analysis of patients (246) with intertrochanteric fractures treated with a sliding compression hip screw at Hennepin County Medical center was performed. Of these 246 fractures, 20 were classified as fractures with a major intertrochanteric component with extension into the femoral neck. These fractures were similar to intertrochanteric fractures type I-IV described by Kyle and Gustilo in demographics, osteoporosis, and surgical treatment, but this new fracture fracture now described as Type V had a statistically higher rate of mechanical complications 0.0001, reoperation 0.0002, and failure of fixation 0.0001. The overall failure rate was 50%. The majority of these fractures were the result of complete collapse of the hip screw. We feel this higher complication and reoperation rate is secondary to inherent instability in the intertrochanteric fracture which extends into the femoral neck. This instability leads to cmplete collapse of a sliding hip screw result ing in a rigid device that leads to failure of fixation. This fracture complex also has a higher rate of nonunion and avascular necrosis. Although rare, this fracture must be recognized in the fracture classification of intertrochanteric fractures because of its poor prognosis. Other forms of treatment than a sliding hip screw may be considered with this fracture complex because of its high failure rate with standard treatment

Aims. The aim of this study was to inform a definitive trial which could determine the clinical effectiveness of the X-Bolt Dynamic Hip Plating System compared with the sliding hip screw for patients with complex pertrochanteric fragility fractures of the femur. Patients and Methods. This was a single centre, participant blinded, randomised, standard-of-care controlled pilot trial. Patients aged 60 years and over with AO/ASIF A2 and A3 type femoral pertrochanteric fractures were eligible. Results. The primary outcome was the EuroQoL 5 Dimension Score (EQ-5D-3L) at one year following index fixation. A total of 100 participants were recruited, and primary outcome data were available for 88 patients following losses to follow-up and withdrawals. The mean difference in EQ-5D was 0.03 (95% confidence interval -0.17, 0.120; p = 0.720.) There were no significant differences in any of the secondary outcomes measures. The recruitment and follow-up rates from this feasibility study were as predicted. Conclusion. A definitive trial with 90% power to find a clinically important difference in EQ-5D would require 964 participants based upon the data from this study. We plan to start recruitment to this trial in Spring 2016. Take home message: A definitive trial of X-Bolt Dynamic Hip Plating System is feasible and should be conducted now in order to quantify the clinical effectiveness of this novel implant. Awareness that minimising tip apex distance (TAD) when inserting a sliding hip screw reduces the risk of screw cut out is important for orthopaedic trainees. The advent of the Picture Archive and Communication System (PACS) has made the accurate measurement of TAD from x-rays and image intensifier films much easier. This study was to determine whether TAD would be reduced if a surgeon knew that his performance was being continually monitored. 31 consecutive cases of sliding hip screw insertion by a single group of surgeons were identified and the TAD measured. The mean TAD was 16.11mm (8.87mm-25.47mm). The same surgeons were then re-educated as to the importance of the TAD and informed that their results would be monitored and discussed. The next 34 consecutive cases were collected prospectively. The mean TAD in these cases was 13.83mm (6.72mm-21.51mm). There was a significant difference between the TAD for the two groups using the two-tailed t-test (p=0.034). There was one implant failure in the pre education group and none in the post education group. These results suggest that awareness of surveillance improves surgical performance even if the importance of the variable being assessed is already known

Controversy exists whether to treat unstable pertrochanteric hip fractures with either intramedullary or extramedullary devices. A prospective randomised control trial was performed to compare the outcome of unstable pertrochanteric hip fractures stabilised with either a sliding hip screw (SHS) or Long Gamma Nail (LGN). The hypothesis was that there is no difference in outcome between the two modes of treatment. Over a four year period, 210 patients presenting with an unstable pertrochanteric hip fracture (AO/OTA 31 A2.1/A2.2/A2.3) were recruited into the study. Eligible patients were randomised on admission to either LGN or SHS. Follow-up was arranged for three, six, and twelve months. Primary outcome measures were implant failure and implant ‘cut-out’. Secondary measures included mortality, length of hospital stay, and EuroQol outcome score. Five patients required revision surgery for implant cutout, of which three were LGNs and two were SHSs (no significant difference). There was a significant correlation between tip apex distance and the need for revision surgery. There were no incidences of implant failure or deep infection. Mortality rates between the two groups were similar when corrected for mini mental score. There was no difference between the two groups with respect to tip apex distance, hospital length of stay, blood transfusion requirement, and EuroQol outcome score. The sliding hip screw remains the gold standard in the treatment of unstable pertrochanteric fractures of the proximal femur

Cerebral micro emboli have been noted to occur during both total hip and knee arthroplasty. These micro emboli have been implicated in the causation of postoperative cognitive impairment. The aim of this study was to determine whether cerebral micro emboli occur during hip fracture surgery. 28 patients undergoing hip fracture surgery had transcranial doppler assessment of the middle cerebral artery to detect cerebral micro emboli. Micro embolic signals (MESs) were recorded during the operative procedure. Successful monitoring was carried out in 26 patients. MES were recorded in 16 out of 26 patients. 12 out of 16 patients who had MESs had undergone a cemented hemiarthroplasty; the remainder had a sliding hip screw for an extracapsular hip fracture. 75% (9/12) of patients who had a cemented hemiarthroplasty had the majority of MESs after reaming and cementing. MESs in the patients who had a sliding hip screw occurred throughout the operative procedure. Conclusion: Cerebral micro emboli do occur during hip fracture surgery. These emboli may be responsible for the cognitive dysfunction that occurs in this susceptible group of patients

A prospective, randomized, controlled trial was performed to compare the outcome of treatment of unstable fractures of the proximal part of the femur with either a sliding hip screw or a short proximal femoral nail antirotation (PFNA-XS, Synthes). Eighty one patients (April 2007 – May 2008) presenting with unstable fracture of the proximal part of the femur were randomized, at the time of admission, to fixation with use of either a short proximal femoral nail antirotation (n=42) or a sliding hip screw (n= 39). The primary outcome measure was reoperation within the first postoperative year and mortality at the end of one year. Operative time, fluoroscopy time, blood loss, and any intra-operative complication were recorded for each patient. Follow-up was undertaken at 3, 6, and 12 postoperative months and yearly thereafter. Plain AP and lateral radiographs were obtained at all visits. All changes in the position of the implant, complications, or fixation failure were recorded. Hip range of motion, pain about the hip and the thigh, walking ability score and return to work status were used to compare the outcomes. There was no significant difference between 1 year mortality rates for the two groups. Mean Operative time was significantly less in PFNA group (Mean 25 min, range 19 – 56 min) than DHS group (Mean 38 min, range 28 – 70 min). Patients treated with a PFNA experienced a shorter fluoroscopy time and less blood loss. 6 patients in DHS group had implant failure as compared to none in PFNA group. The functional outcome was also better in PFNA group. When compared to DHS, PFNA-XS provides better functional outcome for unstable trochanteric fractures with less operative time, less blood loss and less complications, however one year mortality rate remains the same

Purpose: The popularity of intramedullary nails (IMN) for trochanteric hip fractures has grown substantially with little supportive evidence that IMN are superior to conventional sliding hip screws (SHS). We aimed to assess the impact of SHS or IMN intramedullary nailing on functional outcomes and rates of re-operation in elderly patients with fractures. Method: We conducted a multi-center, pilot randomized trial including three clinical sites across Sweden, Denmark, and Canada. We randomized 85 elderly patients with stable and unstable trochanteric hip fractures to either SHS or an IMN. The primary outcome, revision surgery, was independently adjudicated at one year. Secondary functional outcomes included the Parker Mobility Score (PMS), the Merle D’Aubigne Score, the Short Form-12 (SF-12) and the Euroquol-5D. Results: Eighty five patients were enrolled. Fifteen patients died prior to the one year follow up. Across treatment groups, patients did not differ in age, gender and fracture type. The overall revision risk was 11.6% (8/69) and did not differ significantly between groups (IMN: 5; SHS: 3). Patients treated with IMN had significantly higher Merle D’Aubigne function subscores at 6 (p=0.01) and 12 months (p=0.05). Gamma3 nails approached significantly higher scores in the Parker mobility score at 6 (p=0.08) and 12 months (p=0.056). Non-significant differences were identified in the SF-12 and Euroquol-5D quality of life measures; however, in both scores, the Gamma3 nailed trended to higher scores than the sliding hip screw. Conclusion: Our findings of early functional gains without increased risk of revision surgery support the increased popularity of IMN for the management of trochanteric hip fractures in elderly patients

Internal fixation is currently the standard of care for Garden I and II femoral neck fractures in the elderly. However, there may be a degree of posterior tilt on the preoperative lateral radiograph above which failure is likely, and primary arthroplasty would be preferred. The purpose of this study was to determine the association between posterior tilt and the risk of subsequent arthroplasty following internal fixation of Garden I and II femoral neck fractures in the elderly. This study represents a secondary analysis of data collected in the FAITH trial, an international multicenter randomized controlled trial comparing the sliding hip screw to cannulated screws in the management of femoral neck fractures in patients aged 50 years or older. For each patient who sustained a Garden I or II femoral neck fracture and had an adequate preoperative lateral radiograph, the amount of posterior tilt was categorized as < 2 0 degrees or ≥20 degrees. Multivariable Cox proportional hazards analysis was used to assess the association between posterior tilt and subsequent arthroplasty during the two-year follow-up period, while controlling for potential confounders. Of the 555 patients in the study sample, posterior tilt was classified as ≥20 degrees for 67 (12.1%) and < 2 0 degrees for 488 (87.9%). Overall, 13.2% (73/555) of patients underwent subsequent arthroplasty in the 24-month follow-up period. In the multivariable analysis, patients with posterior tilt ≥20 degrees had a significantly increased risk of subsequent arthroplasty compared to those with posterior tilt < 2 0 degrees (22.4% (15/67) vs 11.9% (58/488), Hazard Ratio (HR) 2.22, 95% confidence interval (CI) 1.24–4, p=0.008). The other factor associated with subsequent arthroplasty was age ≥80 (p=0.03). In this study of patients with Garden I and II femoral neck fractures, posterior tilt ≥20 degrees was associated with a significantly increased risk of subsequent arthroplasty. Primary arthroplasty should be considered for Garden I and II femoral neck fractures with posterior tilt ≥20 degrees, especially among older patients

Introduction. We implemented an exhaustive operative and supervision algorithm for surgical treatment of hip fractures primarily based on own previously published literature. The purpose was to improve supervision and reduce the rate of reoperations. Materials and methods. 2000 consecutive unselected patients above 50 years admitted with a hip fracture were included, 1000 of these prospectively after implementation of the algorithm. Demographic parameters, hospital treatment and reoperations within the first postoperative year were assessed from patient records. The algorithm dictated the surgical treatment based on three objective patient parameters: age, new mobility score and fracture classification on pre-operative anterior-posterior and axial radiographs. Intra capsular fractures were treated with two parallel implants, a sliding hip screw, an arthroplasty or resection of the femoral head. Extra capsular fractures were treated with a sliding hip screw or an intramedullary nail. Supervision of junior registrars was mandatory for the prosthesis and intramedullary nail procedures. Results. 931/1000 operative procedures were operated according to the algorithm, compared to only 726/1000 prior to its introduction (p<0.001). Retrospectively we found that 13% (208/1657) of operative procedures performed as the algorithm dictated were reoperated compared to 28% (96/343) of operative procedures performed with other methods (p<0.001). In logistic regression analysis combining sex, age, ASA score, cognitive function, new mobility score and level of surgeon's experience, not following the algorithm was a predictor for re-operation (p<0.001 log. reg.). After implementing the algorithm, junior registrars still performed half of the operations, but unsupervised procedures declined from 192/1000 to 105/1000 (p=0.039). The rate of reoperations declined from 18% to 12% (p<0.001, log. reg.), with a 24% (112/467) to 18% (87/482) decline for intra capsular fractures (p=0.025) and a 13% (68/533) to 7% (37/518) decline for extra capsular fractures (p=0.002). The extra bed-days caused by reoperations were hereby reduced from 24% to 18% of total hospitalization. Conclusion. An exhaustive algorithm for hip fracture treatment can be implemented. In our case, the algorithm both raised the rate of supervision and reduced the rate of reoperations, the latter saving many hospital bed-days

Background: Hip fractures are common among the aged population, with high mortality and morbidity rates. It ‘s annual cost in the United States is expected to double by the year 2040 to about 16 billion U.S Dollars. Of those, approximately 50% are inter-trochanteric fractures. Among them, 50 to 60% are categorized as unstable fractures. Unstable intertrochanteric fractures are defined as 1) fractures with comminution of the posteromedial buttress which exceeds a simple lesser trochanteric fragment; 2) fractures with evidence of subtrochanteric fracture lines; and 3) reverse oblique fractures of the femoral neck. Review of the literature reveals large variations in the amount of complications after surgical treatment of unstable intertrochanteric fractures, among various medial institutes. Infection rates winds from fewer than 1% and up to 15% of cases, and reports of cutout events range from % to 20%. Other complications, such as non-unioin, femoral shaft fractures, and painful hardware, are much less common. Purpose: To investigate the rate of complications after surgical treatment of unstable inter-trochanteric fractures, in our department. Method: Retrospective review of 61 patients who were admitted in our department due to unstable intertro-chanteric fractures, after simple falls, between May 2001 to August 2006, and were treated with intramedullary sliding hip screw. Most of the hardware (90%) were A.O nails (PFN, proximal femoral nail). Results: There were 4 cases of infections, which are 4.9% of cases. Three of them required removal of the hardware. One admission was due to superficial surgical wound infection. There were 3 cases of mechanical cutout of the femoral head screw, which are 6.5% of the cases. No cases of non-union, femoral shaft fractures, or painful hard are noted. Conclusions: To our experience, intramedullary sliding hip screw is a safe and effective treatment for unstable intertrochanteric fractures. Complication rates to our experience are at the lower third compared to reports from medical institutes over the world

Intracapsular neck of femur fractures may be treated with fixation or arthroplasty, depending on fracture characteristics and patient factors. Two common methods of fixation are the sliding hip screw, with or without a de-rotation screw, and cannulated screws. Each has its merits, and to date there is controversy around which method is superior, with either method thought to risk avascular necrosis of the femoral head (AVN) rates in the region of 10–20%. Fixation with cannulated screws may be performed in various ways, with current paucity of evidence to show an optimum technique. There are a multitude of factors which are likely to affect patient outcomes: technique, screw configuration, fracture characteristics and patient factors. We present a retrospective case series analysis of 65 patients who underwent cannulated screw fixation of a hip fracture. Electronic operative records were searched from July 2014 until July 2019 for all patients with a neck of femur fracture fixed with cannulated screws: 68 were found. Three patients were excluded on the basis of them having a pathological fracture secondary to malignancy, cases were followed up for 2 years post-operatively. Electronic patient records and X-rays were reviewed for all included patients. All X-rays were examined by each team member twice, with a time interval of two weeks to improve inter-observer reliability. 65 patients were included with 2:1 female to male ratio and average age of 72 years. 36 patients sustained displaced fractures and 29 undisplaced. Ten patients sustained a high-energy injury, none of which developed AVN. Average time to surgery was 40 hours and 57 patients mobilised on day one post-operatively. All cases used either 7 or 7.3mm partially threaded screws in the following configurations: 2 in triangle apex superior, 39 triangle apex inferior, 22 rhomboid and 2 other, with 9 cases using washers. All reductions were performed closed. Five (8%) of our patients were lost to follow-up as they moved out of area, 48 (74%) had no surgical complications, seven (11%) had mild complications, three (5%) moderate and two (3%) developed AVN. Both of these sustained displaced fractures with low mechanism of injury, were female, ASA 2 and both ex-smokers. One received three screws in apex inferior configuration and one rhomboid, neither fixed with washers. Our AVN rate following intracapsular hip fracture fixation with cannulated screws is much lower than widely accepted. This study is under-powered to comment on factors which may contribute to the development of AVN. However, we can confidently say that our practice has led to low rates of AVN. This may be due to our method of fixation; we use three screws in an apex inferior triangle or four screws in a rhomboid, our consultant-led operations, closed reduction of all fractures, or our operative technique. We pass a short thread cannulated screw across the least comminuted aspect of the fracture first in order to achieve compression, followed by two or three more screws (depending on individual anatomy) to form a stable construct. Our series shows that fixation of intracapsular hip fractures with cannulated screws as we have outlined remains an excellent option. Patients retain their native hip, have a low rate of AVN, and avoid the risks of open reduction

Purpose: Minimizing tip-apex distance (TAD) has been shown to reduce clinical failure of extramedullary sliding hip screws used to fix peritrochanteric fractures. There is debate regarding the optimal position of the lag screw in the femoral head when a cephalomedullary nail is used to treat a peritrochanteric fracture. Some authors suggest the TAD should be minimized as with an extramedullary sliding hip screw, while others suggest the lag screw should be placed inferior within the femoral head. The primary goal of this study was to determine which of 5 possible lag screw positions in the femoral head provides greatest mechanical stiffness and/or load-to-failure for an unstable peritrochanteric fracture treated with a cepha-clomedullary nail. The secondary goal was to determine if there is a linear correlation between implant-femur mechanical stiffness and/or load to failure (dependent variables) with a series of five radiographic measurements (independent variables) of distance from the lag screw tip to the femoral head apex. Method: Long Gamma 3 Nails (Stryker, Mahwah, NJ) were inserted into 30 left synthetic femurs (Pacific Research Laboratories, Vashon, WA). An unstable four-part fracture was created, anatomically reduced, and repaired using one of 5 lag screw placements in the femoral head:. superior (n=6),. inferior (n=6),. anterior (n=6),. posterior (n=6),. central (n=6). All specimens were radiographed in the anterioposterior and lateral planes, and radiographic measurements including TAD and a calcar referenced tip-apex distance (CalTAD) were calculated. All specimens were tested for axial, lateral, and torsional stiffness, and then loaded-to-failure in the axial position using an Instron 8874 (Canton, MA). ANOVA was used to compare means of the five treatment groups. Linear regression analysis was used to compare stiffness and load-to-failure (dependant variables) with radiographic measurements (independent variables). A post hoc power analysis was performed. Results: The inferior lag screw position had significantly greater mean axial stiffness than superior (p< 0.01), anterior (p=0.02) and posterior (p=0.04) positions. Analysis revealed significantly less mean torsional stiffness for the superior lag screw position compared to other lag screw positions (p< 0.01 all 4 pairings). No statistical differences were noted for lateral stiffness. Superior and central lag screw positions had significantly greater mean load-to-failure than anterior (p< 0.01 and p=0.02) and posterior (p< 0.01 and p=0.05) positions. There were significant negative linear correlations between stiffness tests with CalTAD, and load-to-failure with TAD. Power was greater than 95% for axial stiffness, torsional stiffness and load-to-failure tests. Conclusion: Position of the lag screw in the femoral head affects the biomechanical properties of the implant-femur construct. Central placement of the lag screw with minimization of TAD may provide the best combination of stiffness and load-to-failure

Few will disagree that the best femoral head that a young patient can have is his or her own, native femoral head. In the active, healthy patient under age 60 with a displaced femoral neck fracture, well-done, timely ORIF presents the best chance of preserving the patient's native femoral head. Arthroplasty is generally reserved for older patients, over age 60, where attempts at ORIF in this setting have demonstrated failure rates over 40%. “Physiologic age” is a somewhat nebulous term that takes into account the health and ambulatory status of the patient. For example, a 52-year-old with end-stage renal failure, severe osteoporosis, and a displaced femoral neck fracture may best be treated with arthroplasty. However, in reality, such situations are quite rare. Recent studies have documented that approximately 80% of young patients with displaced femoral neck fractures treated with ORIF will keep their own femoral head for 10 years after injury. The variables under the surgeon's control include timing of fixation, quality of reduction, accurate implant placement and implant selection, and capsulotomy. All of these variables potentially affect outcomes. Fractures in this young age group are frequently high shear angle (vertical) Pauwels type 3 fractures, and benefit from fixed angle fixation. The author prefers anatomic reduction and stabilization with a sliding hip screw and a superiorly placed derotation screw. Careful attention to detail is important to obtain an anatomic reduction, which is the most important variable in the outcome of these challenging injuries

Background. This clinical study aims to establish the rate of operative inadvertent hypothermia (IH) in elderly hip fracture patients (>65 years old). We postulate that differences exist in risk factors and hypothesised poorer outcomes in patients with IH. Methods. A single centre, retrospective study of 929 hip fracture patients managed operatively between June 2015 and July 2017 was conducted. Patients’ demographic, anaesthetic and surgical variables were analysed together with outcomes for length of stay (LOS), 30-day re-admissions, and 30-day mortality. Results. Overall rates of IH in elderly hip fracture patients undergoing surgery were 10%, with increasing age as a risk factor (p = 0.005). There was trend towards IH in patients receiving sliding hip screw (SHS) (p = 0.079). No difference in LOS was observed between IH and normothermic patients (8.9 ± 7.1 versus 8.6 ± 4.9, p= 0.51). 30-day re-admissions were 18.5% for IH patients versus 7.8% in normothermic patients (p<0.001). There was a trend towards a higher 30-day mortality (p = 0.089), and a significantly higher mortality in IH patients undergoing SHS (p = 0.014). Conclusion. Rates of IH are high in operatively managed hip fracture patients. IH is significantly associated with a higher 30-day readmission rate with a trend towards higher 30-day mortality. This study mandates an examination of strategies for maintaining normothermia in operatively managed elderly hip fracture patients

In a randomised trial involving 598 patients with 600 trochanteric fractures of the hip, the fractures were treated with either a sliding hip screw (n = 300) or a Targon PF intramedullary nail (n = 300). The mean age of the patients was 82 years (26 to 104). All surviving patients were reviewed at one year with functional outcome assessed by a research nurse blinded to the treatment used. The intramedullary nail was found to have a slightly increased mean operative time (46 minutes (. sd. 12.3) versus 49 minutes (. sd. 12.7), p < 0.001) and an increased mean radiological screening time (0.3 minutes (. sd. 0.2) versus 0.5 minutes (. sd. 0.3), p <  0.001). Operative difficulties were more common with the intramedullary nail. There was no statistically significant difference between implants for wound healing complications (p = 1), or need for post-operative blood transfusion (p = 1), and medical complications were similarly distributed in both groups. There was a tendency to fewer revisions of fixation or conversion to an arthroplasty in the nail group, although the difference was not statistically significant (nine versus three cases, p = 0.14). The extent of shortening, loss of hip flexion, mortality and degree of residual pain were similar in both groups. The recovery of mobility was superior for those treated with the intramedullary nails (p = 0.01 at one year from injury). In summary, both implants produced comparable results but there was a tendency to better return of mobility for those treated with the intramedullary nail

Background. Isolated fracture of the greater trochanter is an uncommon presentation of hip fracture. Traditional teaching has been to manage these injuries nonoperatively, but modern imaging techniques have made it possible to detect occult intertrochanteric extension of the fracture in up to 90% of cases. This study aims to review the investigation and management of greater trochanter fractures in a single major trauma centre. Methods. A retrospective review was completed of patients admitted with greater trochanter fractures. These were matched to cases with 2-part extracapsular fractures. Initial management and clinical outcome was established using electronic notes and radiographs. Mortality and length of stay was calculated for both groups. Results. 85 isolated greater trochanter fractures in 84 patients were identified from 2006–2017. 81/85 patients were treated non-operatively. 78 were mobilised full weight bearing. None required readmission or operation due to fracture displacement. 58 of these patients had cross-sectional imaging with MRI or CT and 15 of those scanned had intertrochanteric extension of the fracture. In the same time period, 998 2-part extra-capsular fractures were treated, using a sliding hip screw. Length of stay was shorter in patients with greater trochanter fractures than 2-part extracapsular fractures (median 7 days vs 14 days, P<0.0001). 30-day mortality was 11.9%, with no significant difference to patients with 2-part extracapsular fractures. Discussion. Cross sectional imaging rarely changed the treatment protocol for isolated greater trochanter fractures. The outcome following non-operative treatment is good even in the presence of occult fractures identified on CT or MRI. We advocate a treatment protocol that encourages early mobilisation and repeat plain radiographs if patients fail to progress. This will reduce unnecessary morbidity from fixation of stable occult fractures

Aims. To compare the outcomes for trochanteric fractures treated with a sliding hip screw (SHS) or a cephalomedullary nail. Patients and Methods. A total of 400 patients with a trochanteric hip fracture were randomised to receive a SHS or a cephalomedullary nail (Targon PFT). All surviving patients were followed up to one year from injury. Functional outcome was assessed by a research nurse blinded to the implant used. Results. Recovery of mobility, as assessed by a mobility scale, was superior for those treated with the intramedullary nail compared with the SHS at eight weeks, three and nine months (p-values between 0.01 and 0.04), the difference at six and 12 months was not statistically significant (p = 0.15 and p = 0.18 respectively). The mean difference was around 0.4 points (0.3 to 0.5) on a nine point scale. Surgical time for the nail was four minutes less than that for the SHS (p < 0.001). Fracture healing complications were similar for the two groups. There were no statistically significant differences between implants for any other recorded outcomes including the need for post-operative blood transfusion, wound healing complications, general medical complications, hospital stay or mortality. Conclusion. This study confirms the findings of a previous study that both methods of treatment produce similar results, although intramedullary fixation does result in marginally improved regain of mobility in comparison with the SHS. Background. The Targon FN plate is a combination of the sliding hip screw and multiple cancellous screws. It is used in the fixation of intracapsular fractures of the neck of femur. The aim of this prospective audit was to assess clinical and radiological outcomes of Targon FN. Method. All patients who had a Targon FN fixation over a period of 18 months at a district general hospital were included. A pro forma was completed using medical records, including x-ray images. Results. Thirty-five patients were identified. Median (IQR) age was 73 (57–82). Median (IQR) waiting time for surgery was 27 hours (17–51). Median (IQR) operating time was 58 (50–65) minutes. The patients were followed up at 6, 12 18 and 24 months. Three cases of avascular necrosis were reported and two cases of non-union. Seven cases were found where the Targon FN was not used correctly. No cases of implant failure were reported where the Targon FN was used according to manufacture guidelines. Five revision surgeries took place or were being planned for cases of avascular necrosis, non union and symptomatic hardware. One case was identified which would have been better treated with a hemiarthroplasty than Targon FN. Conclusion. We recommend that the Targon FN plate continue to be used in our department. The success rate of the implant could be improved by educational workshops in our department to ensure that all surgeons adhere strictly to the operating technique described by the manufacturer. We recommend continuing careful selection of patients for Targon FN and to continue a follow up to 24 months

Objectives. The Sliding Hip Screw (SHS) is commonly used to treat trochanteric hip fractures. Fixation failure is a devastating complication requiring complex revision surgery. One mode of fixation failure is lag screw cut-out which is greatest in unstable fracture patterns and when the tip-apex distance of the lag screw is > 25 mm. The X-Bolt Dynamic Hip Plating System (X-Bolt Orthopaedics, Dublin, Ireland) is a new device which aims to reduce this risk of cut-out. However, some surgeons have reported difficulty minimising the tip-apex distance with subsequent concerns that this may lead to an increased risk of cut-out. Patients and Methods. We measured the tip-apex distance from the intra-operative radiographs of 93 unstable trochanteric hip fractures enrolled in a randomised controlled trial (Warwick Hip Trauma Evaluation, WHiTE One trial). Participants were treated with either the sliding hip screw or the X-Bolt dynamic hip plating system. We also recorded the incidence of cut-out in both groups, at a median follow-up time of 17 months. Results. There was a significantly increased tip-apex distance with the use of the X-Bolt (mean difference 3.7mm (95% confidence interval 1.58 to 5.73); SHS mean 17.1 mm, X-Bolt mean 20.8; p = 0.001. However, this was not associated with an increased incidence of cut-out at a median follow-up time of 17 months, with three cut-outs (6%) in the SHS group and 0 (0%) in the X-Bolt group. Conclusion. The X-Bolt is a safe implant with no increased risk for cut-out. Concerns about minimising the tip-apex distance may be justified but do not appear to be clinically important. Cite this article: M. Femoral head collapse due to avascular necrosis (AVN) is a relatively rare occurrence following intertrochanteric fractures; however, with over thirty-thousand intertrochanteric fractures per year in England and Wales alone, and an incidence of up to 1.16%, it is still significant. Often patients are treated with a hip fixation device, such as a sliding hip screw or X-Bolt. This study aimed to investigate the influence of three factors on the likelihood of head collapse: (1) implant type; (2) the size of the femoral head; and (3) the size of the AVN lesion. Finite element (FE) models of an intact femur, and femurs implanted with two common hip fixation designs, the Compression Hip Screw (Smith & Nephew) and the X-Bolt (X-Bolt Orthopaedics), were developed. Experimental validation of the FE models on 4. th. generation Sawbones composite femurs (n=5) found the peak failure loads predicted by the implanted model was accurate to within 14%. Following validation on Sawbones, the material modulus (E) was updated to represent cancellous (E=500MPa) and cortical (E=1GPa) bone, and the influence of implant design, head size, and AVN was examined. Four head sizes were compared: mean male (48.4 mm) and female (42.2 mm) head sizes ± two standard deviations. A conical representation of an AVN lesion with a lower modulus (1MPa) was created, and four different radii were studied. The risk of head collapse was assessed from (1) the critical buckling pressure and (2) the peak failure stress. The likelihood of head collapse was reduced by implantation of either fixation device. Smaller head sizes and greater AVN lesion size increased the risk of femoral head collapse. These results indicate the treatment of intertrochanteric fractures with a hip fixation device does not increase the risk of head collapse; however, patient factors such as small head size and AVN severity significantly increase the risk

The Radiographic Union Score for Hip (RUSH) is an outcome instrument designed to describe radiographic healing of femoral neck fractures. The ability to identify fractures that have not healed is important for defining non-union in clinical trials and predicting patients that likely require additional surgery to promote fracture healing. We sought to determine a RUSH threshold score that defines nonunion at 6-months post-injury. Our secondary objective was to determine if this threshold was associated with increased risk for non-union surgery. A sample of 248 patients with adequate six-month hip radiographs and complete two-year clinical follow-up were analysed from a multi-national hip fracture trial (FAITH). All patients had a femoral neck fracture and were treated with either multiple cancellous screws or a sliding hip screw. Two reviewers independently determined the RUSH score based on the six-month post-injury radiographs, and agreement was assessed using the Interclass Correlation Coefficient (ICC). Fracture healing was determined by two independent methods: 1) prospectively by the treating surgeon using clinical and radiographic assessments, and 2) retrospectively by a Central Adjudication Committee using radiographs alone. Receiver Operator Curve analysis was used to define a RUSH threshold score that was specific for fracture nonunion. RUSH score inter-rater agreement was high (ICC: 0.81, 95% CI 0.76 to 0.85). The mean six-month RUSH score for all included patients was 24.4 (SD 3.4). A threshold score of <18 was associated with a greater than 98% specificity for nonunion. Furthermore, patients with a six-month RUSH score below 18 were more the seven-times more likely to require revision surgery for nonunion (Relative Risk: 7.25, 95% CI 2.62 to 20.00). The six-month RUSH score can effectively be used to communicate when a femoral neck fracture has not healed. The validity of our conclusions was further supported by the increased risk of nonunion surgery for patients below the RUSH threshold. We believe our findings can standardise a definition of nonunion for clinical trials and recommend the use of the RUSH and its <18-point threshold when describing femoral neck nonunion

Radiation exposure is a hazard to orthopaedic surgeons, theatre staff and patients intra-operatively. Obesity is becoming a more prevalent problem worldwide and there is little evidence how a patient's body habitus correlates with the radiation doses required to penetrate the soft tissues for adequate imaging. We aimed to identify if there was a correlation between Body Mass Index (BMI) and radiation exposure intra-operatively. We performed a retrospective review of 75 patients who underwent sliding hip screw fixation for femoral neck fractures in one year. We recorded Body Mass Index (BMI), screening time, dose area product (DAP), American Society of Anesthesiologists (ASA) grade, seniority of surgeon and complexity of the fracture configuration. We analysed the data using statistical tests. We found that there was a statistically significant correlation between dose area product and patient's BMI. There was no statistically significant relationship between screening time and BMI. There was no statistical difference between ASA grade, seniority of surgeon, or complexity of fracture configuration and dose area product. Obese patients are exposed to increased doses of radiation regardless of length of screening time. Surgeons and theatre staff should be aware of the increased radiation exposure during fixation of fractures in obese patients and, along with radiographers, ensure steps are taken to minimise these risks

Introduction and Aims. Peritrochanteric femur fractures are common and impose major costs on the healthcare system. A fixed angle sliding hip screw is the principle method of treatment, but the rate of mechanical failure associated with these devices can be high; the usual mechanism being the collapse of the neck shaft angle leading to extrusion of the screw from the femoral head, commonly known as “cutout”. Many variables contribute to the risk of cutout, however there is substantial evidence that the “Tip Apex Distance” (a single number that summarises the position and depth of the lag screw on anteroposterior and lateral radiographs after controlling for magnification) is the single best predictor of risk of failure. There is a strong statistical relationship between an increasing tip apex distance (TAD) and the rate of cutout. Specifically, a TAD of 25 mm or less significantly reduces risk of failure. The aim of our study is to assess levels of compliance with a maximum TAD of 25 mm in peritrochanteric femur fractures treated with Dynamic Hip Screws and IM Nails. Methods. We retrospectively measured the TAD of 45 patients who had undergone DHS or IM nails in the previous 6 months. Results. 21/28 of DHS were within 25 mm as opposed to 15/17 of IM nails. Discussion. In this cohort, 75% and 88% of patients respectively had a tip apex distance which complied with best evidence to reduce risk of failure. In total, 20% of patients fell outwith acceptable limits. It was surmised that the IM nails would have greater tip apex distances due to fractures generally being more complex, however it is suggested that this may be ameliorated by greater senior input in these cases

We conducted a prospective randomised trial to compare the results of anatomical reduction and medial displacement osteotomy in 127 consecutive patients with unstable intertrochanteric fractures, of whom 109 completed the study. After an average follow-up of 11 months, we found no significant differences in walking ability, social status or failure of fixation in the two groups. Postoperative complication rates and the early mortality rate were not significantly different, but operating time and blood loss were significantly higher in the osteotomy group. With the use of modern sliding hip screws, medial displacement osteotomy is rarely indicated for unstable intertrochanteric fractures

Background: The Hansson Twin Hook (HTH) is an alternative to the sliding hip screw in the treatment of trochanteric fractures. In osteoporotic bone, biomechanical tests indicate better fixation properties of the HTH than of the lag screw. Our aim was to evaluate the technical results of the HTH in a larger series of osteoporotic patients with intertrochanteric fractures. Many surgeons were involved to assess, if the device was user-friendly. Patients and Methods: In a prospective bicentric study, 55 surgeons used the HTH and a standard plate in 157 consecutive patients with intertrochanteric fractures, of which 83% were unstable. The mean age of the patients was 83 years. The patients were followed regularly clinically and radiographically for at least 4 months with a final control at 2 years. Results: Technical intraoperative errors were done in 7 of the patients. The reduction of the fracture was inaccurate in these cases; hence the HTH had not been placed centrally in the femoral head. Two of the 7 intraoperative errors developed into failures of fixation (1.3%) during the 2-year period. Interpretation: The HTH achieves adequate fixation purchase in osteoporotic bone, has a low failure rate and is easy to use

Fractures of the proximal femur at the level of the lesser trochanter (reversed and transverse fracture lines, Evans classification type II, AO classification A3 fractures) are known to have an increased risk of fixation failure. 58 patients with such a fracture were randomised to have the fracture fixed with either an intramedullary nail (220 mm Targon PF nail) or a sliding hip screw (SHS). The mean age of the patients was 82 and 11% were male. All patients were followed up for one year by a research nurse blinded to the treatment groups. Mean length of surgery was 50 minutes for the nail versus 52 minutes for the SHS. There were no differences between groups in the need for blood transfusion. Operative complications tended to be less for the nail group (1/27 versus 5/26). Mean hospital stay was 18 days for the nail group versus 29 days for those treated with the SHS. The only fracture healing complications were one case of cut-out in the SHS group and two cut-outs in the nail group, two of which required revision surgery. During follow-up those patients treated with the nail reported a tendency to lower pain scores than those treated with the SHS (p=0.04 at two months). This difference persisted even at one year from injury. Mortality and regain of mobility was similar between groups. These results indicate that for these difficult fractures types both types of fixation produce comparable outcomes

In a prospective study we assessed the causes of mechanical failure in a series of 230 intertrochanteric femoral fractures which had been internally fixed with either a sliding hip screw or a Kuntscher Y-nail. The overall rate of mechanical failure was 16.5%; cutting-out of the implant from the femoral head was the cause in three-quarters of the instances. Implants placed posteriorly in the femoral head cut out more often (27%) than those placed centrally (7%). The cut-out rate was also determined by the quality of the fracture reduction, but age, walking ability and bone density (assessed by the Singh grade and metacarpal indices) had no significant influence. We conclude that these fractures should be reduced as accurately as possible and it is imperative that the implant is placed centrally within the femoral head

Early and safe mobilization remains the mainstay of treatment for the intertrochanteric fractures. Many implants have been used but the intramedullary implants seem to provide maximum stability in unstable fractures. The aim of this study was to compare two intramedullary devices which have different philosophy and modes of action. Between 01.07.2005 to 30.06.2007, 110 patients with trochanteric fractures treated with the intramedullary hip screw (IMHS, Smith & Nephew) nail and 105 with the ENDOVIS (Citieffe) nail. The data recorded were the fracture’s type, preoperative hemoglobin level, pre-fracture walking ability. Postoperatively were recorded the operative time, the level of hemoglobin on the first day, mobility status, complications and 12 months mortality rate. The results were comparable regarding blood loss, operative time, and mortality rate. However, the rehabilitation and functional outcome were superior in the IMHS group. This probably was attributed to the higher number of complications of the ENDOVIS nail. IMHS is a valuable tool for the treatment of stable intertrochanteric fractures, offering also excellent results in the management of reverse obliquity, comminuted fractures and those with a subtrochanteric extension. Further investigations are necessary to prove which the ideal intramedullary implant is. However, it seems that devices combining the principles of the sliding hip screw with those of an intramedullary nail present safe and accurate fixation, fewer mechanical failures and exceptional functional outcomes

The Medoff sliding plate (MSP) is a new device used to treat intertrochanteric and subtrochanteric fractures. There are three options for sliding; either along the shaft or the neck of the femur, or a combination of both. In a prospective series of 108 consecutive displaced intertrochanteric fractures we used combined dynamic compression. The patients were followed clinically and radiologically for one year. All fractures healed during the follow-up period. The only postoperative technical failure was one lag-screw penetration. Combined compression of the MSP gives increased dynamic capacity which reduces the risk of complications. The low rate of technical failure in our series compares favourably with that of the sliding hip screw or the Gamma nail but randomised trials comparing the MSP with other hip screw systems are necessary to find the true role of the MSP with its various sliding modes

Osteotomy has been used in the treatment of unstable intertrochanteric hip fractures in an attempt to increase the stability of the fracture fragments. We have assessed this stability in a randomised prospective trial on 100 consecutive patients, all having fixation by an AO dynamic hip screw, comparing anatomical reduction with two types of osteotomy. The groups were similar in terms of age, gender, mental test score, and fracture configuration. There were more failures of fixation in the osteotomy groups, and the operations took longer. We found no clear benefit from osteotomy and therefore recommend anatomical reduction and fixation by a sliding hip screw in most cases. Rarely, a fracture configuration which does not allow load-sharing between the fracture fragments and the device may benefit from an osteotomy or the use of an alternative implant

Background: Aim of the study was to evaluate the clinical results of Montgomery Hip Screw for fixation of proximal femoral osteotomies. There are a number of devices for proximal femoral fixation, including sliding hip screws. Rotational instability of the proximal femoral segment can be a problem. To overcome this, a hip screw has been introduced with two screws in the proximal segment. Methods and Results: A prospective cohort undergoing osteotomy was followed up. Inclusion criteria included consecutive patients < 16 years of age, with an indication for elective proximal femoral osteotomy. All operations were performed by senior author or under his supervision using a standard postero-lateral approach. Further incisions for adductor/psoas release and pelvic osteotomy were added as indicated. In 23 cases MHS was used with a mean follow up of 10 months (6 to 24). In 9 patients there was an underlying neurological problem, one case of LCPD, and the rest had DDH. Previous surgery with a hip screw on the contralateral side had been undertaken in 5 cases. The mean age was 5 years (range 1 to 12) and mean time to union was 6.3 weeks. There were no occurrences of rotational instability or failure of fixation. No wound complication was encountered in the cohort. Conclusions: Early results indicate that Montgomery Hip Screw is a safe device for fixation of proximal femoral osteotomy with the added advantage of rotational stability

Introduction. Intracapsular hip fractures in young adults are treated with internal fixation, often as a surgical emergency to reduce the vascular insult to the femoral head. Avascular necrosis and non-union often require revision surgery. Methods. A prospective trauma database was retrospectively searched from 1. st. August 2008 to 31. st. December 2010. Intracapsular hip fracture admissions to the Regional Trauma Centre, in adults under sixty-five years, were studied. Ninety-eight intracapsular fractures internally fixed with sliding hip screw underwent radiographic review for quality of fixation, and evidence of complications at a mean of thirteen months. Co-morbid medical conditions and quality of fixation were assessed for association with revision rates. Results. Avascular necrosis developed in eighteen fractures, and seven proceeded to non-union. Seventeen of ninety-eight fractures underwent revision to total hip arthroplasty. Three individuals were deceased at final review. Six of thirty-four fractures (18%) fixed within twelve hours of injury required revision, compared with eleven of sixty-four (17%) fixed after twelve hours. None of the four fractures fixed within six hours required revision, although this was not statistically significant (p=0.32). Revision was required for three of six individuals with chronic respiratory disease (50%) excluding asthma, one of two with rheumatoid arthritis (50%), and three of six with varus reduction (50%), and each were significant on multiple regression. Residual translation and eccentric screw placement approached significance on chi-squared analysis. Only two of twenty (10%) fractures fixed using an additional anti-rotation screw required revision. Conclusions. Internal fixation of intracapsular hip fractures within twelve hours in young adults did not statistically reduce revision rates. Accurate reduction and internal fixation is paramount, and anti-rotation screws may reduce complication rates. Certain co-morbid conditions predict revision, and should be taken into consideration when selecting the primary operative procedure

This multicentre audit assessed the total Tip Apex Distance (TAD) of sliding hip screws for intertrochanteric hip fractures in the 3 fracture hospitals in Northern Ireland (Ulster Hospital, Royal Victoria Hospital & Altnagelvin Hospital). Patient demographics and anaesthetic information was also reviewed. A sample of 140 patients with adequate screening films (39 UHD, 50 RVH & 51 Altnagelvin) were selected. The TAD was measured on AP & lateral screening films and compared to the standard of 25mm or less (total in 2 views) as recommended by Baumgaertner et al (JBJS (Am) 1995). All 3 hospitals had an average of under 25mm (22.1, 19.9 & 19.6mm respectively) with overall average of 20.4mm, and a TAD of 25mm or less was achieved in 66.7%, 82% & 80.4% in respective hospitals (77.1% of patients overall). No patients were readmitted due to cut-out, despite 22.9% of patients having a TAD greater than 25mm. Among patients with TAD over 25mm the average TAD was 30.1mm. Demographics showed a 77.8% of patients to be female, with a slight predominance of left sided injuries. Most patients were of ASA grading 2–3. Anaesthetic method preferences varied between hospitals. Patients with TAD over 25mm were not significantly different from those with TAD of 25mm or less in age, gender, ASA or operated side


400 patients with a trochanteric hip fracture were randomised to fixation with either a 220mm long Targon PF (proximal femoral) nail or a Sliding Hip Screw. All surgery was undertaken or supervised by one surgeon. All patients were followed up for a minimum of one year by a blinded observer. The mean age was 82 years (range 27 to 104 years), 20% were male. Mean length of surgery was slightly increased for the nail (44 versus 49 minutes, p=0.002). Fluoroscopic screening time was increased in the nail group (0.3 versus 0.6 minutes, p< 0.0001). Intra-operative complications were more common with the nailing. There was no difference in blood transfusion requirement between groups. Postoperatively there was no difference in the occurrence of medical complications or mortality. Deep wound infection requiring removal of the implant occurred in one case in the SHS group. In addition there were two cases of cut-out, three of plate detachment from the femur and one non-union in the SHS group, requiring secondary surgery. There were only once compilations in the nailed a case of cut-out which required secondary surgery. At follow-up there was no difference in pain scores between groups but there was a tendency to improved mobility in the nailed group (p=0.004). These results suggest that with improved designs and surgical technique, the newer versions of short nails for proximal femoral fractures may not suffer from the complications of the earlier short intramedullary nails. Intramedullary fixation can result in a lower re-operation rate (3.5% versus 0.5%) and improved mobility in comparison to the sliding hip screw

The last decade has seen a rise in the use of the gamma nail for managing inter-trochanteric and subtrochanteric hip fractures. Patients with multiple co-morbidities are under high anaesthetic risk of mortality and are usually not suitable for general or regional anaesthesia. However, there can be a strong case for fixing these fractures despite these risks. Apart from aiming to return patients to their pre-morbid mobility, other advantages include pain relief and reducing the complications of being bed bound (e.g. pressure ulcers, psychosocial factors). While operative use of local anaesthesia and sedation has been documented for insertion of extra-medullary femoral implants such as the sliding hip screw, currently no literature is present for the insertion of the gamma nail. We studied intra-operative and post-operative outcomes of three patients aged between 64 and 83 with right inter-trochanteric hip fractures and American Society of Anesthesiologists (ASA) scores of 4 or more. Consent for each case was obtained after discussion with the patient and family, or conducted with the patient's best interests in mind. All patients received a short unlocked gamma nail, and were operated on within 24 hours of admission. Each patient underwent local injections of Bupivacaine or Lignocaine or both, with Epinephrine, and with one patient receiving nerve block of the fascia iliaca. Each patient received a combination of sedatives under the discretion of the anaesthetist including Midazolam, Ketamine, Propofol, Fentanyl, and/or Haloperidol. Operating time ranged from 30–90 minutes. Patients were managed post-operatively with analgesia based on the WHO pain ladder and physiotherapy. Our results showed no intra-operative complications in any of the cases. All patients noted improvement in their pain and comfort post-operatively without complications of the operation site. Two patients achieved their pre-morbid level of mobility after undergoing physiotherapy and were subsequently discharged from the orthopaedic team. One patient with ongoing pre-operative medical complications died 5 days after the operation. This study provides a glimpse into the use of local anaesthetic and sedation on high operative risk patients, and this may be a viable alternative to extra-medullary implants or non-operation. Further research is needed to quantify the risks and benefits of operating within this patient group

We examined the effect of the Gamma nail on strain distribution in the proximal femur, using ten cadaver femora instrumented with six unidirectional strain gauges along the medial and lateral cortices. The femora were loaded to 1800 N and strains were determined with or without distal interlocking screws before and after experimentally created two-part and four-part fractures. Motion of the sliding screw and the nail was also determined. Strain patterns and screw motion were compared with previously obtained values for a sliding hip screw device (SHS). The Gamma nail was shown to transmit decreasing load to the calcar with decreasing fracture stability, such that virtually no strain on the bone was seen in four-part fractures with the posteromedial fragment removed; increasing compression was noted, however, at the proximal lateral cortex. Conversely, the SHS showed increased calcar compression with decreasing fracture stability. The insertion of distal interlocking screws did not change the pattern of proximal femoral strain. The Gamma nail imparts non-physiological strains to the proximal femur, probably because of its inherent stiffness. These strains may alter bone remodelling and interfere with healing. Distal interlocking screws may not be necessary for stable intertrochanteric fractures

In a prospective, consecutive study we reviewed the results of 32 supracondylar femoral fractures treated by Smith & Nephew intramedullary supracondylar nailing between January 1996 and October 2002. The mean age of 23 women and nine men was 67 (58 to 89). All fractures were closed. Two patients had associated upper limb fractures. In four cases, fractures occurred around total knee prostheses. Four patients had previously undergone ipsilateral total hip replacement or had had a sliding hip screw. On the AO classification the majority of fractures were type-33A1 and A2; seven fractures were classified as C1 and C2. The patients were placed in the supine position on a radiolucent operating table with the knee in 30° of flexion. Postoperatively a hinged knee brace was applied and worn until union. Supported, progressive knee movement was introduced from day one. Partial weight-bearing was permitted as soon as pain subsided and continued until there were radiological signs of union. Within three to six months all but two fractures united. These united after prolonged bracing. There was no sepsis or fixation failure. A functional range of knee movement was observed in all patients. Retrograde intramedullary nailing is a safe and successful method of management of supracondylar femoral fractures in the elderly and offers a minimally invasive alternative

Introduction. Total knee arthroplasty is effective for the management of osteoarthritis of the knee. Conventional techniques utilizing manual instrumentation (MI) make use of intramedullary femoral guides and either extramedullary or intramedullary tibial guides. While MI techniques can achieve excellent results in the majority of patients, those with ipsilateral hardware, post-traumatic deformity or abnormal anatomy may be technically more challenging, resulting in poorer outcomes. Computer-assisted navigation (CAN) is an alternative that utilizes fixed trackers and anatomic registration points, foregoing the need for intramedullary guides. This technique has been shown to yield excellent results including superior alignment outcomes compared to MI with fewer outliers. However, studies report a high learning curve, increased expenses and increased operative times. As a result, few surgeons are trained and comfortable utilizing CAN. Patient-specific instrumentation is an alternative innovation for total knee arthroplasty. Custom guide blocks are fabricated based on a patient's unique anatomy, allowing for the benefits of CAN but without the increased operative times or the high learning curve. In this study we sought to evaluate the accuracy of PSI techniques in patients with previous ipsilateral hardware of the femur. Methods. After reviewing our database of 300 PSI total knee arthroplasty patients, 16 were identified (10 male, 6 female) using the Zimmer NexGen Patient Specific Instrumentation System. Fourteen patients included in the study had a preexisting total hip arthroplasty on the ipsilateral side [Figure 1], 1 had a sliding hip screw, and 1 patient had a cephalomedullary nail. Postoperative mechanical axis alignment measurements were performed using plain long-standing radiographs [Figure 2]. The American Knee Society Score was used to evaluate clinical outcomes postoperatively. Results. Sixteen total knee arthroplasties were performed using PSI, all in the setting of previous ipsilateral hardware placement. The average age at the time of surgery was 72, with patients ranging from 56 to 85 years of age [Table 1]. 11 of the included knees had a preoperative varus alignment and 5 had valgus alignment. The average value of a deformity identified via the preoperative planning software was 7.9°(1.5°–15.7°). The average value of a deformity identified via preoperative radiographs was 10.1°(2.2°–14.7°). Average postoperative mechanical axis was 3.1° (1°–5.3°) measured from plain radiographs. Average angle between the FMA and femoral component was 90.0° (85.3°–94.1°). The average angle between the TMA and tibial component was 90.6°(87.6°–92.9°). The average difference between the femoral mechanical and anatomic axes was 5.9°(3.4°–7.0°). The average discrepancy between medial and lateral joint space on an anterior-posterior standing radiograph was 0.4mm(0.0mm–1.1mm). At an average of 4.5 months follow-up, American Knee Society knee scores show an aggregate average score of 82.94. Conclusions. Patient specific instrumentation (PSI) is an innovative technology in TKA that replaces the use of intramedullary femoral guides and either extramedullary or intramedullary tibial guides. This study demonstrates that PSI is capable of producing favorable radiographic and clinical outcomes despite preexisting ipsilateral hardware, which may otherwise preclude the use of customary manual instrumentation. We believe PSI is an accurate and effective tool for use in patients with preexisting ipsilateral hardware

Aims

To investigate if preoperative CT improves detection of unstable trochanteric hip fractures.

Methods

A single-centre prospective study was conducted. Patients aged 65 years or older with trochanteric hip fractures admitted to Stavanger University Hospital (Stavanger, Norway) were consecutively included from September 2020 to January 2022. Radiographs and CT images of the fractures were obtained, and surgeons made individual assessments of the fractures based on these. The assessment was conducted according to a systematic protocol including three classification systems (AO/Orthopaedic Trauma Association (OTA), Evans Jensen (EVJ), and Nakano) and questions addressing specific fracture patterns. An expert group provided a gold-standard assessment based on the CT images. Sensitivities and specificities of surgeons’ assessments were estimated and compared in regression models with correlations for the same patients. Intra- and inter-rater reliability were presented as Cohen’s kappa and Gwet’s agreement coefficient (AC1).

PURPOSE: The treatment of pertrochanteric fractures is constantly evolving and surgical issues remain controversial. Although the use of a sliding hip screw is considered to be the treatment of choice by many surgeons, we believe that intramedullary nailing could be a viable treatment option for unstable pertrochanteric fractures. Given this background, we compared the clinical outcome of unstable pertrochanteric fractures treated with intramedullary hip screw (IMHS) and standard screws with unstable pertrochanteric fractures treated with intramedullary hip screw using hydroxyapatite (HA)-coated screws. METHODS: Forty consecutive female osteoporotic patients with pertrochanteric fractures were selected. The inclusion criteria were: female; age ≥65 years; pertrochanteric fracture resulting from minor trauma. Patients were randomized by a computer-generated list to receive either IMHS fixed with stainless steel lag screws (Group A) or IMHS fixed with HA-coated pins (Group B). RESULTS: Average patient age was 82 ± 8 years in Group A and 78 ± 6 years in Group B. Average BMD was 512 ± 177 in Group A, and 471 ± 231 in Group B. Average intraoperative time was 64 ± 6 minutes in Group A and 34 ± 5 minutes in Group B (p < 0.005). In Group A, all patients had post-operative blood transfusions averaged 2.0 ± 0.1. In Group B, there were no blood transfusions (p < 0.0001). In Group A, the reduction over time in the femoral neck-shaft angle was 6 ± 8, while in Group B, the reduction was 2 ± 1 (p < 0.002). Conclusion: This study showed that intramedullary hip screw with HA-coated lag screw is an effective treatment for unstable fractures in this patient population. The operative time is brief, the fixation is adequate, and the reduction is maintained over time

Hip fracture treatment strategies continue to evolve with the goal of restoring hip fracture victims to Pre-injury Functional levels. Strategies for improved treatment have focused on fracture exposure, reduction, provisional fixation and definitive fixation with implant designs optimised for fracture union with minimal implant failure as originally proposed by Lambotte. Multiple implant designs have been conceived based on perceived inadequacies of previous generational designs. To better understand this evolutionary process, it is necessary to review the predecessors of modern fracture treatment and understand their design concepts and results. It is interesting that the modern era of surgical treatment of hip fractures actually began in 1902, when Dr Royal Whitman advocated the necessity of a closed reduction of adult hip fractures under general anesthesia and stabilisation by hip spica cast. Dr Whitman predicted the evolution of stabilisation by internal fixation and commented on this in his 1932 JBJS editorial emphasising the importance of surgical treatment of fractures. Dr Smith-Peterson, also from New York, in 1925 developed the 1st commercially successful hip implant, a tri–flanged nail. These first surgeries were performed with an open reduction, through a Smith-Petersen approach without radiographic control. This nail device was rapidly modified in the 1930's to permit insertion over a guide wire with a radiographic controlled insertion technique, a minimally invasive procedure. Nail penetration and implant failure in pertrochanteric fractures led to the rapid development of side-plates and a refocus on reduction stability. This led to a period of primary corrective osteotomies for enhanced stability, but fell out of failure after the sliding hip screw concept took hold. Originally conceived by Godoy-Moreira and Pohl independently in the 1940s, it became rapidly accepted as a method to avoid nail penetration and implant failure, unfortunately at the expense of accepting malunion and collapse of the fracture. Even the importance of rotational stability was discarded as insignificant by Holt in 1963. The concept of reduction of the Antero-Medial cortex was forgotten in favour of the Tip-apex distance as the only important variable in reduction to avoid implant cut-out. The concept of malunion of pertrochanteric fractures was simply deleted from consideration with disregard for the possible association of impaired functional recovery. Several recent papers that improved functional recovery is possible when these new implants are coupled with successful reduction strategies. Further studies are needed to identify the correct choice of implant for the appropriate fracture configuration, which may lead to a revision of our current fracture classification systems and our concepts of stability

There is continuing debate regarding the merits of internal fixation versus arthroplasty for intracapsular hip fractures. For displaced fractures there is a higher risk of fracture healing complications but many of the studies reporting on this are now from some years ago using surgical methods that are now outdated. The aim of our study was to determine the incidence of avascular necrosis (AVN) related to age, gender, displacement and operation delay for a large series of patients with intra-capsular fractures treated with internal fixation using contemporary methods. We prospectively studied 1023 patients, 988 fractures were internally fixed with three cannulated screws and 35 with a sliding hip screw. The average length of radiographic follow-up for those patients who survived to one year was 337 days (range 25 to 3521 days). The average age of the patients was 75.4 years (range 16–100); 243 (23.8%) were male. The overall incidence of AVN was 6.6%. AVN was less common (p=0.0004) for undisplaced fractures than for displaced fractures (21 of 528 [4.0%] vs 47 of 495 [9.5%]) and in men than women (p=0.03) who had a displaced fracture (7 of 143 [4.9%] vs 40 of 352 [11.4%]). The incidence of AVN for those aged less than 60 years and who sustained a displaced fracture was 20.6%, compared to 12.5% for those aged 60–80 years and 2.5% for those aged more than 80 years (p< 0.0001). We found no association between the incidence of AVN and operation delay. Our study showed an increased risk of AVN with younger age and in females with a displaced fracture. This is in contrast to the decreased incidence of non-union seen with younger age. Knowledge of the predicted incidence of fracture healing complications should help surgeons make a more balanced decision between internal fixation and arthroplasty for this condition

Introduction: The Ionising Radiations Medical Exposure Regulations Act 2000 has established diagnostic reference levels for radiological examinations, however at present there are no national guidelines available for orthopaedic trauma surgery. There may be a role for the introduction of diagnostic reference levels at a local level therefore we studied patient area dose and screening time for orthopaedic trauma operations performed in the Regional Trauma Centre in Northern Ireland. Methods: Retrospectively data was retrieved from written radiography records in the Royal Victoria Hospital, during the period of 1st January 2007 to 31st December 2007 for all orthopaedic trauma cases in which an image intensifier was used. The screening time, patient area dose (PAD), details of the operation, patient age, sex, month of the operation and grade of the operating surgeon (trainee or consultant), were recorded. Results: 1709 cases using image intensifier were reviewed. 137 cases were excluded due to incomplete data. 319 hips were screened for insertion of sliding hip screw, mean screening time was 0.51min with a mean PAD of 145cGycm2. 127 femoral nails were inserted with an average screening time of 1.84min and mean PAD of 310cGycm2. 166 tibias were screened for application of Ilizarov frame or insertion of tibial nail, average screening time was 3.00min with a mean PAD of 48cGycm2. 129 spinal cases were screened with an average screening time of 0.80mins and mean PAD of 37.9cGycm2. Consultants had lower screening times and mean PADs than trainees with 0.63min versus 1.01min and 65.8cGycm2 versus 70.9cGycm2. Conclusions: The average screening times and mean PADs compared favourably with local reference guides for image intensifier cases and with other published series. Every trauma unit should have local reference ranges for orthopaedic imaging cases and regular audit should be carried out to assess compliance with these ranges

A neck of femur fracture is known to be a high risk factor for the development of pressure sores with an associated morbidity, mortality and cost. We have attempted to identify risk factors in these patients for the development of pressure sores. We have analysed prospectively collected data of 4654 consecutive patients (1003 males/3473 females). 3.8% developed pressure sores in the sacral, buttock or heel areas. Patients factors that increased the risk of pressure sores were increased age (82.1 years versus 76.6 years), lower mental test score (5.7 versus 6.7), diabetes mellitus (pressure sore incidence 9.4%), higher ASA score (3.0 versus 2.7) and lower admission haemoglobin concentration (120gms versus 124gms). Those patients with an extracapsular fracture were more likely to develop pressure sores compared to patients with an intracapsular fracture (4.5% versus 3.1%). Being male was not a risk factor. Among surgical factors related to an increased risk was a fall in blood pressure during surgery (5.6%). Patients who underwent a dynamic hip screw were more likely to develop pressure sores (pressure sore incidence 4.7%). Patients with an intracapsular fracture treated with internal fixation were less likely to develop pressure sores in comparison to those fractures treated with a hemiarthroplasty or a sliding hip screw (2.0% versus 4.7 versus 4.4%). No relationship was seen related to length of surgery of type of anaesthesia. Our study indicates that the current incidence of pressure sores is lower that that previously reported (30%). Whilst it is possible in a large population of patients to determine factors that increase the risk of pressure sores, these are not sufficiently reliable to be used for an individual patient

Patients with an intracapsular fracture treated with internal fixation were less likely to develop pressure sores in comparison to those fractures treated with a hemiarthroplasty or a sliding hip screw (2.0% versus 4.7 versus 4.4%). No relationship was seen related to length of surgery or type of anaesthesia. Our incidence of pressure sores is lower than previously reported (30%). Whilst determining factors that increase the risk of pressure sores may not be sufficiently reliable to be used for the individual patient, taking appropriate preventative measures can reduce the incidence, particularly with reference to (optimising the patient pre-operatively and) reducing delays to surgery

Background: A radiological audit of the local use of the Dynamic Hip Screw in extracapsular proximal femur fractures. Study aim: to identify cases of mechanical failure and revision, to determine predictors of fixation failure. Methods: A retrospective radiological review of 567 consecutive cases at Western Australian tertiary hospitals over a 3 year period (2002 – 2004) using the Picture Archive Computer System (PACS). Results: Female: male ratio was 2.79: 1. Evan’s classification: 418 fractures stable (73.7%), 149 unstable (26.3%). Failure of fixation occurred in 14 cases (2.5%); ten due to hip screw cut out (1.8%) and four due to plate pull off (0.8%). All cases of cut out had a significantly higher mean tip apex distance (TAD) (31 vs 20mm, P < 0.001) and an unstable fracture configuration; 8 of 10 had a poor reduction. Bivariate logistical regression revealed TAD of 25mm or more to be most predictive of cut out; followed by mean TAD, superior anterior and inferior posterior screw placement, unstable fracture configuration and poor reduction. Unassociated factors included gender, age, American Society of Anesthesiologists’ score, plate angle and length, operation time and surgeon level. A three-variable model found TAD of 25mm or more and unstable fracture configuration to be predictive, but not poor reduction. Cases with a TAD of 25mm or more with unstable fracture configuration and a poor reduction had a 21.6% chance of cut out (8 of 29). Conclusions: This is the first multifactorial multivariate analysis of a single implant sliding hip screw series. Compared with the literature, the rate of failure is low. Possible reasons include appropriate choice of implant for fracture type, improved performance with use of a single model of implant, and low exclusion rates due to the use of PACS

Unstable intertrochanteric fractures may be treated by several types of implants, most frequently by dynamic sliding hip screw or some form of intramedullary implant. Intramedullary implants began to be used in cases with an expectation of further improvement of osteosynthesis stability. A need to determine the advantages of single implants for selected types of fractures in randomized trials was defined. In addition to biomechanical principles, bone quality is considered, together with increasing possibilities in recent years of further improving density measurements, especially qCT with respect to local specificity. A series of 86 patients (24 men, 62 women, average age 77,6 years) was operated on from September 6, 2005 to June 30, 2009 for unstable intertrochanteric fracture (31 A2.1, A2.2, A2.3), either by DHS of PFN osteosynthesis after randomization. A CT examination of both hip joints in a predefined manner was performed before surgery. Using special software the relative density of the central spherical part of the femoral head 2 and 3 centimetres in diameter was determined. After fracture healing, the dynamization of the neck screw of both implants and the reduction of vertical distance between the tip of the neck screw and subchondral bone of the femoral head were determined. In addition to evaluation of osteosynthesis stability and osteosyntheis failure, clinical parameters such as surgical time, blood loss and length of hospital stay were compared between the two groups of patients. Survival of patients was evaluated with respect to April 21, 2010. In the patient series, 4 failures of DHS osteosynthesis (cut out) and 2 failures of PFN osteosynthesis (cut out) were noted. Sliding of the DHS was on average 11,9 mm, and was significantly higher in comparison to dynamization of the PFN neck screw, which was 6,9 mm (p=0,005). When comparing the vertical distance between the tip of the neck screw and subchondral bone of the femoral head immediately after surgery and after fracture healing the average reduction of the vertical distance was 1,6 mm in DHS osteosynthesis and 0,8 mm in PFN osteosynthesis. The difference was statistically significant (p=0,025). PFN seems to provide a more stable fixation, based on the measurements. The number of failed DHS osteosyntheses is higher in comparison to the number of failed PFN osteosyntheses but the difference is not statistically significant. The influence of femoral head density on osteosynthesis failure could not be determined due to a low number of failed osteosyntheses in both patient groups. At the same time, after statistical analysis, influence of the relative femoral head density on vertical distance reduction between the screw tip and femoral head subchondral bone in healed fractures was not proven. Statistically, average length of surgical time, length of hospital stay, mean blood loss and survival did not differ significantly between the two patient groups

Background: The most commonly used implant for the internal fixation of an extracapsular proximal femoral fracture is a sliding hip screw (SHS). More recently short intramedullary nails (IMN) have been advocated as an alternative, particularly for unstable fractures due to possible biomechanical advantages. The purpose of this meta-analysis was to compare, on the basis of evidence from randomised controlled trials, the fixation outcome with these two types of implant in stable and unstable fractures. Method: All randomised controlled studies comparing intramedullary nails with a SHS were considered for inclusion. Studies were identified using the search strategy of the Cochrane Collaboration, with no restriction on languages or source. Two authors independently extracted the data, and assessed trial methodology. Results: 24 randomised trials involving 3202 patients with 3279 fractures were included in the analysis. Pooled results gave no statistically significant difference in the cut-out rate between the IMN or SHS 41/1556 and 37/1626 (Relative risk 1.19; 95% confidence interval 0.78 to 1.82). There was an increased total failure rate (103/1495 and 58/1565, Relative risk 1.83; 95% confidence interval 1.35 to 2.50) and re-operation rate (57/1357 and 35/1415, Relative risk 1.63; 95% confidence interval 1.11 to 2.40) with the IMN compared the SHS when all fractures were considered. Fracture healing complications were much less frequent for stable fractures. No evidence for a reduced failure rate for IMN’s in unstable fractures patterns could be found. Conclusions: The results from studies to date indicate an increased fixation failure rate for trochanteric fractures fixed with an intramedullary nail, and show no benefit to the use of a nail in unstable fractures. Therefore the use of intramedullary nails for trochanteric fractures cannot be recommended

Introduction and Aims: Compression hip screws are considered to be the gold standard for treatment of trochanteric proximal femoral fractures. Two implant designs exist; the ‘keyed’ and ‘non-keyed’ barrel profiles. Many biomechanical studies have been published on the performance of sliding hip screws, but most have used only static testing, and none to our knowledge have sought to compare the two-barrel profiles. This study aimed to compare the sliding characteristics of keyed and non-keyed systems in both static and dynamic loading conditions. Method: Tests were performed on the implants using a multi axis servo-hydraulic testing machine. The machine possessed both linear and torsional actuators, such that hip flexion/extension could be simulated during testing. Load to initiate sliding in both implants was measured in a variety of testing conditions; screw engagement in barrel (20–38mm), angle of hip flexion (0–40 degrees), perpendicular loading force (50–190N), and cycle frequency (0–1 Hz). Results: Results showed a tendency towards greater sliding in the non-keyed system, although these were significant only for the screw engagement testing (p< 0.001). However, load to initiate sliding in both implants was significantly higher in dynamic as compared to static testing (p< 0.001), and increased as torsional frequency increased. The non-keyed system did not demonstrate any tendency for screw rotation within the barrel during dynamic testing. Conclusion: The non-keyed compression hip screw system does show a trend towards improved sliding characteristics, and does not display the tendency for screw rotation within the barrel under loading, often quoted as a misgiving of this implant. Also, since forces to initiate sliding are significantly higher when these implants are loaded dynamically (which mimics more closely the in vivo performance), future biomechanical studies should include dynamic testing for any hip fracture implant

Introduction:. Total knee arthroplasty (TKA) is an effective operation for the management of osteoarthritis of the knee. Conventional technique utilizing manual instrumentation (MI) allows for reproducible and accurate execution of the procedure. The most common techniques make use of intramedullary femoral guides and either extrameduallary or intrameduallary tibial guides. While these methods can achieve excellent results in the majority of patients, those with ipsilateral hardware, post-traumatic deformity or abnormal anatomy may preclude the accurate use of these techniques. Patient-specific instrumentation (PSI) is an alternative innovation for total knee arthroplasty. Utilizing magnetic resonance imaging (MRI) or computed tomography (CT), custom guide blocks are fabricated based on a patient's unique anatomy. This allows for the benefits of computer assisted navigation (CAN) but without the increased operative times or the high learning curve associated with it. Furthermore it allows the use of familiar cutting blocks and guides to check the accuracy of the PSI guide blocks. In this study we sought to evaluate the accuracy of PSI techniques in patients with previous ipsilateral hardware, which would make the use of MI technically challenging and possibly subject to inaccuracy. Methods:. After reviewing our database of 300 PSI total knee arthroplasty patients, 16 patients were identified (10 male, 6 female) using the Zimmer NexGen Patient Specific Instrumentation System. Fourteen patients included in the study had a preexisting total hip arthroplasty on the ipsilateral side, 1 had a preexisting sliding hip screw, and 1 patient had a preexisting cephalomedullary nail. Postoperative mechanical axis alignment measurements were performed using plain long-standing radiographs. The American Knee Society Score was used to evaluate clinical outcomes postoperatively. Results:. Sixteen total knee arthroplasties were performed using PSI, all in the setting of previous ipsilateral hardware placement. The average age at the time of surgery was 72, with patients ranging from 56 to 85 years of age. Eleven of the included knees had a preoperative varus alignment and 5 had valgus alignment. The average value of a deformity identified via the preoperative planning software was 7.85°. The average value of a deformity identified via preoperative radiographs was 10.1°. Average postoperative mechanical axis was 3.1° measured from plain radiographs. Average angle between the femoral mechanical axis and femoral component was 90.0°. The average angle between the tibial mechanical axis and tibial component was 90.6°. The average difference between the femoral mechanical and anatomic axes was 5.9°. The average discrepancy between medial and lateral joint space on an anterior-posterior standing radiograph was 0.4 mm. At an average of 4.5 months follow-up, American Knee Society knee scores show an aggregate average score of 82.94. Conclusion:. Patient specific instrumentation is an innovative technology in TKA that replaces the use of intramedullary femoral guides and either extramedullary or intramedullary tibial guides. This study demonstrates that PSI is capable of producing favorable radiographic and clinical outcomes despite preexisting ipsilateral hardware, which may preclude the use of customary manual instrumentation. We believe PSI is an accurate and effective tool for use in patients with preexisting ipsilateral hardware

Introduction. The sliding hip screw is the implant of choice for the operative treatment of stable trochanteric femur fractures. Surgeons have been using widely the four-hole side plate DHS (Dynamic Hip screw) with four bicortical screws, which allows adequate weight bearing after operation. However, there is lacking of scientific studies that support the use of such long plate and we question ourselves if we can accomplish the same results with the use of a smaller plate. The objective of this study is to compare the results accomplished with a four-hole and a two-hole DHS side plate in the treatment of transtrochanteric fractures. Material and Methods. This study included 140 patients (43 male and 97 female) that had stable transtrochanteric fractures between 1/01/2005 and 31/12/2008 and were submitted to osteossynthesis with DHS side-plate. 32 (22.9%) were treated with a two-hole DHS (group DHS2) and 108 (77.1%) with four-hole. The fractures were evaluated according to the AO/OTA classification and Evans for stability. The fracture reduction was assessed according to Sernbo criteria and was recorded also patient demographics, fracture patterns and fixation, comorbilities, mortality rate, capacity of ambulation and complications. Results. The patients had in medium 77.74 ± 49.52 years and 18 months of follow-up (range 6–36 months). Both groups had similar patient demographics. The etiology of the fracture was fall in 120 (85.7%) and 20 (14.3%) from traffic accident, 10 (7.1%) were patological. 15 (10.7%) died during hospital stay: 13 (12.0%) in DHS4 group and 2 (6.3%) in DHS2. In terms of capacity of ambulation in the group DHS2 15.6% didn't ambulate and 25% had walking aid; in the group DHS4 20.4% didn't ambulate and 29.7% had walking aid. Concerning fracture reduction there was varus (<125°) in 9.4% of DHS2 group and 9.3% in DHS4. Also in the group DHS4 there were 15 (13.9%) complications: 3 cut-out, 3 device failure, 8 infections and 1 pseudarthrosis. In the group DHS2 there were 4 (12.5%) complications: 1 cut-out, 2 infections and 1 device failure. 121 (28 group DHS2 and 93 group DHS4) fractures healed without complications in anatomical position with good function of the hip joint. Discussion. We found no significant differences between the two groups regarding reduction or percentage of complications. However we could observe that in the group DHS2 there was a lesser rate of mortality during hospital stay and a higher capacity of ambulation without walking aid. So the fixation of stable transtrochanteric fractures with a two-hole DHS side-plate is safe, less invasive, less surgical time and less blood loss than a four-hole. As our study reveled in these stable fractures there is lacking of benefit with the use of a larger slide-plate, the two-hole is adequate and its use should be increasing in our clinical practice

At Bispebjerg University Hospital in Denmark, an Optimized Hip Fracture Program, (OHP) is the standard of care for all hip fracture patients. Part of OHP is pain treatment with a femoral nerve catheter placed at the emergency room, dosed 4 times a day with Bupivacain until 4 days after surgery, combined with systemic analgesics as needed. In 2008, a database and a bio bank were created at the ward including all hip fracture patients to make a better description of the population and as a tool for further optimisation of the OHP. One of the aims was to identify possible subgroups having specific complications, which could lead to a differentiation of the OHP by markers known at the time of admission. In this analysis, we will focus on postoperative pain that inhibits mobilization by fracture type and type of surgery. Methods. A consecutive cohort of 898 hip fracture patients hospitalized within two years from September 2008 to July 2010 was used for this study. Patients coming from nursing homes were excluded from the study, since nearly all are discharged 1 or 2 days after surgery to rehabilitation. If the exact type of fracture was not known, or if the patient was admitted for rehabilitation after surgery at another hospital, the patient was also excluded. 508 patients were thus included in the study. Mean age 80.0 years (SD:23.7), 72.1% women and 27.9% men. Mobilization inhibited by postoperative pain as documented in the patient files was registered in the database. Overall mobilization was inhibited by pain in 26.7% of the 508 patients included. The following fracture types were studied: Femoral neck Garden 1–2, Femoral neck Garden 3–4, Pertrochanteric Evans 1–2, Pertrochanteric Evans 3–5, Basocervical and Subtrochanteric. Results. Mobilization inhibited by pain was not significantly associated with type of surgery:. Cannulated Hip Screws 16/80 (20.0%), sliding hip screw 23/94 (24.5%), Intramedullary Nail 63/186 (33.9%), partial hip replacement 33/144 (22.9%), total hip replacement 1/5 (20.0%). P = 0.09 chi square. On the other hand mobilization inhibited by pain was significantly associated with fracture type:. Femoral neck Garden 1–2: 13/64 (20.3%), Femoral neck Garden 3–4: 38/172 (22.1%), Pertrochanteric Evans 1–2: 18/69 (26.1%), Pertrochanteric Evans 3–5: 46/149 (30.9%), Basocervical: 6/28 (21.4%), Subtrochanteric 15/27(55.6%). P = 0.009 chi square. Conclusion. The fracture types Pertrochanteric Evans 3–5 and Subtrochanteric had the biggest inhibition of mobilization caused by pain. No significant association between type of surgery and inhibition of mobilization caused by pain was found

The gamma interlocking nail, designed combining the advantages of the sliding hip screw with the intramedullary nail, was initially introduced for the management of unstable proximal femoral fractures. However the unacceptably high incidence of lateral femoral shaft fractures led to the development of the long gamma nail. This is the result of a prospective study of the use of the long gamma nail in 35 patients over a 7 year period till March 2000. The mean age of the patients was 69.9 years. There were 13 men and 22 women. All but two of the fractures had a subtrochanteric component. Ten were pathological fractures. An identical size of nail was used in all cases. Elderly patients were permitted to mobilise without restriction, whereas partial weight bearing was imposed on the younger patients till some signs of radiological healing. Patients were reviewed at a hip fracture clinic. Mean clinical follow up was 381 days and radiological follow up was 244 days. Mean hospital stay 22 days. The post operative mortality at 30 days was 20%, rising to 45% at one year. General complications that occurred were pneumonia – 3, fat embolism – 1, myocardial infarction – 1, and GI bleed – 1. Four cases had superficial wound infection, which resolved with oral antibiotics. Fracture related complications occurred in 4 cases. These were intra-operative femoral shaft fracture – 1, fracture at tip of nail – 1, nail breakage – 2. All went on to heal after exchange nailing. The long gamma nail does not appear to have reduce the post-operative incidence of femoral fractures, which is most likely related to the large size of the distal locking screws and stress concentration at the tip of the nail. The two cases of nail breakage appear to reflect metal fatigue failure in the setting of delayed union in younger patients

Minimizing tip-apex distance has been shown to reduce clinical failure of sliding hip screws used to fix peritro-chanteric fractures. The purpose of this study was to determine if such a relationship exists for the position of the lag screw in the femoral head using a cephalomedullary device. Methods: Thirty intact synthetic femur specimens (Model #3406, Pacific Research Laboratories, Vashon, WA) were potted into cement blocks distally for testing on an Instron 8874 (Instron, Canton, MA). A long cephalomedullary nail (Long Gamma 3 Nail, Stryker, Mahwah, NJ) was inserted into each of the femurs. An unstable four-part fracture was created, anatomically reduced, and repaired using one of 5 lag screw placements in the femoral head:. Superior (N=6),. Inferior (N=6),. Anterior (N=6),. Posterior (N=6),. Central (N=6). Mechanical tests were repeated for axial, lateral and torsional stiffness. All specimens were radiographed in the anterioposterior and lateral planes and tip-apex (TAD) distance was calculated. A calcar referenced tip-apex distance (CalTAD) was also calculated. ANOVA was used to compare means of the five treatment groups. Linear regression analysis was used to compare axial, lateral and torsional stiffness (dependant variables) to both TAD and CalTAD (independent variables). Results: ANOVA testing proved that the mean axial (p< 0.01) and torsional stiffness (p< 0.01) between the 5 groups was significantly different, but lateral stiffness was not statistically different (p=0.494). Post hoc analysis showed that the inferior lag screw position provided significantly higher mean axial stiffness (568.14±66.9N/ mm) than superior (428.0±45.6N/mm; p< 0.01), anterior (443.2±45.4N/mm; p=0.02) and posterior (456.7±69.3N/ mm; p=0.04) lag screw positions. There was no significant difference in mean axial stiffness between inferior (568.14±66.9N/mm) and central (525.4±81.7N/mm) lag screw positions (p=0.77). Post hoc analysis revealed significantly less mean torsional stiffness for the superior lag screw position compared to other lag screw positions (p< 0.01 all 4 pairings). There were no significant correlations between TAD and axial (r=−0.33, p=0.08), lateral (r=−0.22, p=0.24) or torsional (r=0.08, p=0.69) stiffness. There were significant correlations between CalTAD and axial (r=−0.66, p< 0.01), lateral (r=−0.38, p=0.04) and torsional (r=−0.38, p=0.04) stiffness. Discussion: Our results suggest that placement of the lag screw inferiorly in the femoral head when using a cephalomedullary nail to treat an unstable peritrochanteric fracture results in the stiffest construct in axial and torsional biomechanical testing. A simple radiographic measurement, CalTAD, provides an intraoperative method of determining optimal cephalomedullary nail lag screw position to achieve greatest construct stiffness

Aims

This work aimed at answering the following research questions: 1) What is the rate of mechanical complications, nonunion and infection for head/neck femoral fractures, intertrochanteric fractures, and subtrochanteric fractures in the elderly USA population? and 2) Which factors influence adverse outcomes?

Methods

Proximal femoral fractures occurred between 1 January 2009 and 31 December 2019 were identified from the Medicare Physician Service Records Data Base. The Kaplan-Meier method with Fine and Gray sub-distribution adaptation was used to determine rates for nonunion, infection, and mechanical complications. Semiparametric Cox regression model was applied incorporating 23 measures as covariates to identify risk factors.

The principle of the sliding hip screw is to provide a controlled collapse at the fracture site. It is during the screw insertion that clockwise rotational torque is imparted to the head and neck. In right-sided fractures the screw causes the head fragment to rotate clockwise leading to apposition or flexed position of the fracture site. In Left sided fractures the clockwise rotation leads to the head and neck fragment into extension of the fracture site leading to a potentially unstable construct. All intertrochanteric fractures treated over a 12-month period were assessed. 75 fractures were included in the study. The fractures were classified according to Tronzo’s classification (Grades I & II – stable; Grades III & IV – unstable). Intraoperative and postoperative films were assessed for rotational abnormalities in the form of an anterior spike of the proximal fragment in left-sided fractures and a flexed position of reduction in right-sided fractures. There were 39 Left sided fractures and 36 Right sided fractures. A rotational abnormality was seen in 11 Left sided fractures compared with none on the right side. All 11 abnormalities were seen in Grade III and IV fractures (2 and 9 respectively). Analysis of results using the Chi-Square test revealed a significant difference (p < 0.001). 3 out the 11 fractures with rotational anterior spike had an implant cut out which needed revision surgery. Compared to stable fractures, the accuracy of reduction determines the final stability in unstable fractures. In these fractures the rotational torque imparted to the proximal head and neck fragment can cause loss of reduction leading to potential failures of fixation. This appears to be greater in left sided fractures where the rotational torque causes the anterior spike which when not butressed inferiorly and medially can lead to a state where the implant cannot control the shear forces at the fracture site. This can then lead to failure of fixation. In right-sided fractures the rotational torque often causes compression of the head and neck fragment into the distal fragment with the creation of an infero- medial butress. The methods of overcoming this problem are with modifications in the technique. Untwisting the last few threads of the screw after insertion could reduce the anterior spike. The application of digital pressure along the anterior neck or the application of a reduction clamp at screw insertion provides counter rotation. The results of this study confirm that the problem of torque at the fracture site is not of considerable importance in stable fractures but is significantly so in unstable left sided fractures. This results in a greater predisposition for potential failure of fixation

Aims

To develop prediction models using machine-learning (ML) algorithms for 90-day and one-year mortality prediction in femoral neck fracture (FNF) patients aged 50 years or older based on the Hip fracture Evaluation with Alternatives of Total Hip arthroplasty versus Hemiarthroplasty (HEALTH) and Fixation using Alternative Implants for the Treatment of Hip fractures (FAITH) trials.

Methods

This study included 2,388 patients from the HEALTH and FAITH trials, with 90-day and one-year mortality proportions of 3.0% (71/2,388) and 6.4% (153/2,388), respectively. The mean age was 75.9 years (SD 10.8) and 65.9% of patients (1,574/2,388) were female. The algorithms included patient and injury characteristics. Six algorithms were developed, internally validated and evaluated across discrimination (c-statistic; discriminative ability between those with risk of mortality and those without), calibration (observed outcome compared to the predicted probability), and the Brier score (composite of discrimination and calibration).

Aims

Factors associated with high mortality rates in geriatric hip fracture patients are frequently unmodifiable. Time to surgery, however, might be a modifiable factor of interest to optimize clinical outcomes after hip fracture surgery. This study aims to determine the influence of postponement of surgery due to non-medical reasons on clinical outcomes in acute hip fracture surgery.

Methods

This observational cohort study enrolled consecutively admitted patients with a proximal femoral fracture, for which surgery was performed between 1 January 2018 and 11 January 2021 in two level II trauma teaching hospitals. Patients with medical indications to postpone surgery were excluded. A total of 1,803 patients were included, of whom 1,428 had surgery < 24 hours and 375 had surgery ≥ 24 hours after admission.
