Aims. Paediatric fractures are highly prevalent and are most often treated with plaster. The application and removal of plaster is often an anxiety-inducing experience for children. Decreasing the anxiety level may improve the patients’ satisfaction and the quality of healthcare. Virtual reality (VR) has proven to effectively distract children and reduce their anxiety in other clinical settings, and it seems to have a similar effect during plaster treatment. This study aims to further investigate the effect of VR on the anxiety level of children with fractures who undergo plaster removal or replacement in the plaster room. Methods. A randomized controlled trial was conducted. A total of 255 patients were included, aged five to 17 years, who needed plaster treatment for a fracture of the upper or lower limb. Randomization was stratified for age (five to 11 and 12 to 17 years). The intervention group was distracted with VR goggles and headphones during the plaster treatment, whereas the control group received standard care. As the primary outcome, the post-procedural level of anxiety was measured with the Child Fear Scale (CFS). Secondary outcomes included the children’s anxiety reduction (difference between CFS after and CFS before plaster procedure), numerical rating scale (NRS) pain, NRS satisfaction of the children and accompanying parents/guardians, and the children’s heart rates during the procedure. An independent-samples t-test and Mann-Whitney U test (depending on the data distribution) were used to analyze the data. Results. The post-procedural CFS was significantly lower (p < 0.001) in the intervention group (proportion of children with no anxiety = 78.6%) than in the control group (56.8%). The anxiety reduction, NRS pain and satisfaction scores, and heart rates showed no significant differences between the control group and the intervention group. Subanalyses showed an increased effect of VR on anxiety levels in young patients, females, upper limb fractures, and those who had had previous plaster treatment. Conclusion. VR effectively reduces the anxiety levels of children in the plaster room, especially in young girls. No statistically significant effects were seen regarding pain, heart rate, or satisfaction scores. Cite this article: Bone Joint J 2024;106-B(7):728–734

Aims. The Ponseti method is the gold standard treatment for congenital talipes equinovarus (CTEV), with the British Consensus Statement providing a benchmark for standard of care. Meeting these standards and providing expert care while maintaining geographical accessibility can pose a service delivery challenge. A novel ‘Hub and Spoke’ Shared Care model was initiated to deliver Ponseti treatment for CTEV, while addressing standard of care and resource allocation. The aim of this study was to assess feasibility and outcomes of the corrective phase of Ponseti service delivery using this model. Methods. Patients with idiopathic CTEV were seen in their local hospitals (‘Spokes’) for initial diagnosis and casting, followed by referral to the tertiary hospital (‘Hub’) for tenotomy. Non-idiopathic CTEV was managed solely by the Hub. Primary and secondary outcomes were achieving primary correction, and complication rates resulting in early transfer to the Hub, respectively. Consecutive data were prospectively collected and compared between patients allocated to Hub or Spokes. Mann-Whitney U test, Wilcoxon signed-rank test, or chi-squared tests were used for analysis (alpha-priori = 0.05, two-tailed significance). Results. Between 1 March 2020 and 31 March 2023, 92 patients (139 feet) were treated at the service (Hub 50%, n = 46; Spokes 50%, n = 46), of whom nine were non-idiopathic. All patients (n = 92), regardless of allocation, ultimately achieved primary correction, with idiopathic patients at the Hub requiring fewer casts than the Spokes (mean 4.0 (SD 1.4) vs 6.9 (SD 4.4); p < 0.001). Overall, 60.9% of Spokes’ patients (n = 28/46) required transfer to the Hub due to complications (cast slips Hub n = 2; Spokes n = 17; p < 0.001). These patients ultimately achieved full correction at the Hub. Conclusion. The Shared Care model was found to be feasible in terms of providing primary correction to all patients, with results comparable to other published services. Complication rates were higher at the Spokes, although these were correctable. Future research is needed to assess long-term outcomes, parents’ satisfaction, and cost-effectiveness. Aims. Due to their radiolucency and favourable mechanical properties, carbon fibre nails may be a preferable alternative to titanium nails for oncology patients. We aim to compare the surgical characteristics and short-term results of patients who underwent intramedullary fixation with either a titanium or carbon fibre nail for pathological long-bone fracture. Methods. This single tertiary-institutional, retrospectively matched case-control study included 72 patients who underwent prophylactic or therapeutic fixation for pathological fracture of the humerus, femur, or tibia with either a titanium (control group, n = 36) or carbon fibre (case group, n = 36) intramedullary nail between 2016 to 2020. Patients were excluded if intramedullary fixation was combined with any other surgical procedure/fixation method. Outcomes included operating time, blood loss, fluoroscopic time, and complications. Fisher’s exact test and Mann-Whitney U test were used for categorical and continuous outcomes, respectively. Results. Patients receiving carbon nails as compared to those receiving titanium nails had higher blood loss (median 150 ml (interquartile range (IQR) 100 to 250) vs 100 ml (IQR 50 to 150); p = 0.042) and longer fluoroscopic time (median 150 seconds (IQR 114 to 182) vs 94 seconds (IQR 58 to 124); p = 0.001). Implant complications occurred in seven patients (19%) in the titanium group versus one patient (3%) in the carbon fibre group (p = 0.055). There were no notable differences between groups with regard to operating time, surgical wound infection, or survival. Conclusion. This pilot study demonstrates a non-inferior surgical and short-term clinical profile supporting further consideration of carbon fibre nails for pathological fracture fixation in orthopaedic oncology patients. Given enhanced accommodation of imaging methods important for oncological surveillance and radiation therapy planning, as well as high tolerances to fatigue stress, carbon fibre implants possess important oncological advantages over titanium implants that merit further prospective investigation. Aims. Advances in treatment have extended the life expectancy of patients with metastatic bone disease (MBD). Patients could experience more skeletal-related events (SREs) as a result of this progress. Those who have already experienced a SRE could encounter another local management for a subsequent SRE, which is not part of the treatment for the initial SRE. However, there is a noted gap in research on the rate and characteristics of subsequent SREs requiring further localized treatment, obligating clinicians to extrapolate from experiences with initial SREs when confronting subsequent ones. This study aimed to investigate the proportion of MBD patients developing subsequent SREs requiring local treatment, examine if there are prognostic differences at the initial treatment between those with single versus subsequent SREs, and determine if clinical, oncological, and prognostic features differ between initial and subsequent SRE treatments. Methods. This retrospective study included 3,814 adult patients who received local treatment – surgery and/or radiotherapy – for bone metastasis between 1 January 2010 and 31 December 2019. All included patients had at least one SRE requiring local treatment. A subsequent SRE was defined as a second SRE requiring local treatment. Clinical, oncological, and prognostic features were compared between single SREs and subsequent SREs using Mann-Whitney U test, Fisher’s exact test, and Kaplan–Meier curve. Results. Of the 3,814 patients with SREs, 3,159 (83%) patients had a single SRE and 655 (17%) patients developed a subsequent SRE. Patients who developed subsequent SREs generally had characteristics that favoured longer survival, such as higher BMI, higher albumin levels, fewer comorbidities, or lower neutrophil count. Once the patient got to the point of subsequent SRE, their clinical and oncological characteristics and one-year survival (28%) were not as good as those with only a single SRE (35%; p < 0.001), indicating that clinicians’ experiences when treating the initial SRE are not similar when treating a subsequent SRE. Conclusion. This study found that 17% of patients required treatments for a second, subsequent SRE, and the current clinical guideline did not provide a specific approach to this clinical condition. We observed that referencing the initial treatment, patients in the subsequent SRE group had longer six-week, 90-day, and one-year median survival than patients in the single SRE group. Once patients develop a subsequent SRE, they have a worse one-year survival rate than those who receive treatment for a single SRE. Future research should identify prognostic factors and assess the applicability of existing survival prediction models for better management of subsequent SREs. Aims. The aim of this study was to determine the impact of hospital-level service characteristics on hip fracture outcomes and quality of care processes measures. Methods. This was a retrospective analysis of publicly available audit data obtained from the National Hip Fracture Database (NHFD) 2018 benchmark summary and Facilities Survey. Data extraction was performed using a dedicated proforma to identify relevant hospital-level care process and outcome variables for inclusion. The primary outcome measure was adjusted 30-day mortality rate. A random forest-based multivariate imputation by chained equation (MICE) algorithm was used for missing value imputation. Univariable analysis for each hospital level factor was performed using a combination of Tobit regression, Siegal non-parametric linear regression, and Mann-Whitney U test analyses, dependent on the data type. In all analyses, a p-value < 0.05 denoted statistical significance. Results. Analyses included 176 hospitals, with a median of 366 hip fracture cases per year (interquartile range (IQR) 280 to 457). Aggregated data from 66,578 patients were included. The only identified hospital-level variable associated with the primary outcome of 30-day mortality was hip fracture trial involvement (no trial involvement: median 6.3%; trial involvement: median 5.7%; p = 0.039). Significant key associations were also identified between prompt surgery and presence of dedicated hip fracture sessions; reduced acute length of stay and both a higher number of hip fracture cases per year and more dedicated hip fracture operating lists; Best Practice Tariff attainment and greater number of hip fracture cases per year, more dedicated hip fracture operating lists, presence of a dedicated hip fracture ward, and hip fracture trial involvement. Conclusion. Exploratory analyses have identified that improved outcomes in hip fracture may be associated with hospital-level service characteristics, such as hip fracture research trial involvement, larger hip fracture volumes, and the use of theatre lists dedicated to hip fracture surgery. Aims

The aim of this study was to investigate the outcomes of arthroscopic decompression of calcific tendinitis performed without repairing the rotator cuff defect.

Methods

A total of 99 patients who underwent treatment between December 2013 and August 2019 were retrospectively reviewed. Visual analogue scale (VAS) and American Shoulder and Elbow Surgeons (ASES) scores were reviewed pre- and postoperatively according to the location, size, physical characteristics, and radiological features of the calcific deposits. Additionally, the influence of any residual calcific deposits shown on postoperative radiographs was explored. The healing rate of the unrepaired cuff defect was determined by reviewing the 29 patients who had follow-up MRIs.

Aims

In the Netherlands, general practitioners (GPs) can request radiographs. After a radiologically diagnosed fracture, patients are immediately referred to the emergency department (ED). Since 2020, the Máxima Medical Centre has implemented a new care pathway for minor trauma patients, referring them immediately to the traumatology outpatient clinic (OC) instead of the ED. We investigated whether this altered care pathway leads to a reduction in healthcare consumption and concomitant costs.

Methods

In this retrospective cohort study, patients were included if a radiologist diagnosed a fracture on a radiograph requested by the GP from August to October 2019 (control group) or August to October 2020 (research group), on weekdays between 8.30 am and 4.00 pm. The study compared various outcomes between groups, including the length of the initial hospital visit, frequency of hospital visits and medical procedures, extent of imaging, and healthcare expenses.

Aims

This study aimed to explore the diagnostic value of synovial fluid neutrophil extracellular traps (SF-NETs) in periprosthetic joint infection (PJI) diagnosis, and compare it with that of microbial culture, serum ESR and CRP, synovial white blood cell (WBC) count, and polymorphonuclear neutrophil percentage (PMN%).

Methods

In a single health centre, patients with suspected PJI were enrolled from January 2013 to December 2021. The inclusion criteria were: 1) patients who were suspected to have PJI; 2) patients with complete medical records; and 3) patients from whom sufficient synovial fluid was obtained for microbial culture and NET test. Patients who received revision surgeries due to aseptic failure (AF) were selected as controls. Synovial fluid was collected for microbial culture and SF-WBC, SF-PNM%, and SF-NET detection. The receiver operating characteristic curve (ROC) of synovial NET, WBC, PMN%, and area under the curve (AUC) were obtained; the diagnostic efficacies of these diagnostic indexes were calculated and compared.

Aims

Acridine orange (AO) demonstrates several biological activities. When exposed to low doses of X-ray radiation, AO increases the production of reactive radicals (radiodynamic therapy (AO-RDT)). We elucidated the efficacy of AO-RDT in breast and prostate cancer cell lines, which are likely to develop bone metastases.

Methods

We used the mouse osteosarcoma cell line LM8, the human breast cancer cell line MDA-MB-231, and the human prostate cancer cell line PC-3. Cultured cells were exposed to AO and radiation at various concentrations followed by various doses of irradiation. The cell viability was then measured. In vivo, each cell was inoculated subcutaneously into the backs of mice. In the AO-RDT group, AO (1.0 μg) was locally administered subcutaneously around the tumour followed by 5 Gy of irradiation. In the radiation group, 5 Gy of irradiation alone was administered after macroscopic tumour formation. The mice were killed on the 14th day after treatment. The change in tumour volume by AO-RDT was primarily evaluated.

Aims

The aim of this study was to determine whether there is a correlation between the grade of humeral osteoarthritis (OA) and the severity of glenoid morphology according to Walch. We hypothesized that there would be a correlation.

Methods

Overal, 143 shoulders in 135 patients (73 females, 62 males) undergoing shoulder arthroplasty surgery for primary glenohumeral OA were included consecutively. Mean age was 69.3 years (47 to 85). Humeral head (HH), osteophyte length (OL), and morphology (transverse decentering of the apex, transverse, or coronal asphericity) on radiographs were correlated to the glenoid morphology according to Walch (A1, A2, B1, B2, B3), glenoid retroversion, and humeral subluxation on CT images.

Aims

The aim of this study was to compare the clinical and radiological outcomes of patients with early-onset scoliosis (EOS), who had undergone spinal fusion after distraction-based spinal growth modulation using either traditional growing rods (TGRs) or magnetically controlled growing rods (MCGRs).

Methods

We undertook a retrospective review of skeletally mature patients who had undergone fusion for an EOS, which had been previously treated using either TGRs or MCGRs. Measured outcomes included sequential coronal T1 to S1 height and major curve (Cobb) angle on plain radiographs and any complications requiring unplanned surgery before final fusion.

Aims

This study aimed to evaluate the accuracy of implant placement with robotic-arm assisted total hip arthroplasty (THA) in patients with developmental dysplasia of the hip (DDH).

Methods

The study analyzed a consecutive series of 69 patients who underwent robotic-arm assisted THA between September 2018 and December 2019. Of these, 30 patients had DDH and were classified according to the Crowe type. Acetabular component alignment and 3D positions were measured using pre- and postoperative CT data. The absolute differences of cup alignment and 3D position were compared between DDH and non-DDH patients. Moreover, these differences were analyzed in relation to the severity of DDH. The discrepancy of leg length and combined offset compared with contralateral hip were measured.

Aims

The primary aim of this study was to determine the rates of return to work (RTW) and sport (RTS) following a humeral shaft fracture. The secondary aim was to identify factors independently associated with failure to RTW or RTS.

Methods

From 2008 to 2017, all patients with a humeral diaphyseal fracture were retrospectively identified. Patient demographics and injury characteristics were recorded. Details of pre-injury employment, sporting participation, and levels of return post-injury were obtained via postal questionnaire. The University of California, Los Angeles (UCLA) Activity Scale was used to quantify physical activity among active patients. Regression was used to determine factors independently associated with failure to RTW or RTS.

Aims

Patients with soft-tissue sarcoma (STS) who undergo unplanned excision (UE) are reported to have worse outcomes than those who undergo planned excision (PE). However, others have reported that patients who undergo UE may have similar or improved outcomes. These discrepancies are likely to be due to differences in characteristics between the two groups of patients. The aim of the study is to compare patients who underwent UE and PE using propensity score matching, by analyzing data from the Japanese Bone and Soft Tissue Tumor (BSTT) registry.

Methods

Data from 2006 to 2016 was obtained from the BSTT registry. Only patients with STS of the limb were included in the study. Patients with distant metastasis at the initial presentation and patients with dermatofibrosarcoma protuberans and well-differentiated liposarcoma were excluded from the study.

Aims

Global literature suggests that female surgical trainees have lower rates of independent operating (operative autonomy) than their male counterparts. The objective of this study was to identify any association between gender and lead/independent operating in speciality orthopaedic trainees within the UK national training programme.

Methods

This was a retrospective case-control study using electronic surgical logbook data from 2009 to 2021 for 274 UK orthopaedic trainees. Total operative numbers and level of supervision were compared between male and female trainees, with correction for less than full-time training (LTFT), prior experience, and time out during training (OOP). The primary outcome was the percentage of cases undertaken as lead surgeon (supervised and unsupervised) by UK orthopaedic trainees by gender.

Aims

Preprint servers allow authors to publish full-text manuscripts or interim findings prior to undergoing peer review. Several preprint servers have extended their services to biological sciences, clinical research, and medicine. The purpose of this study was to systematically identify and analyze all articles related to Trauma & Orthopaedic (T&O) surgery published in five medical preprint servers, and to investigate the factors that influence the subsequent rate of publication in a peer-reviewed journal.

Methods

All preprints covering T&O surgery were systematically searched in five medical preprint servers (medRxiv, OSF Preprints, Preprints.org, PeerJ, and Research Square) and subsequently identified after a minimum of 12 months by searching for the title, keywords, and corresponding author in Google Scholar, PubMed, Scopus, Embase, Cochrane, and the Web of Science. Subsequent publication of a work was defined as publication in a peer-reviewed indexed journal. The rate of publication and time to peer-reviewed publication were assessed. Differences in definitive publication rates of preprints according to geographical origin and level of evidence were analyzed.

Aims

As the world continues to fight successive waves of COVID-19 variants, we have seen worldwide infections surpass 100 million. London, UK, has been severely affected throughout the pandemic, and the resulting impact on the NHS has been profound. The aim of this study is to evaluate the impact of COVID-19 on theatre productivity across London’s four major trauma centres (MTCs), and to assess how the changes to normal protocols and working patterns impacted trauma theatre efficiency.

Methods

This was a collaborative study across London’s MTCs. A two-month period was selected from 5 March to 5 May 2020. The same two-month period in 2019 was used to provide baseline data for comparison. Demographic information was collected, as well as surgical speciality, procedure, time to surgery, type of anaesthesia, and various time points throughout the patient journey to theatre.

Aims

The aim of this study was to investigate whether on-demand removal (ODR) is noninferior to routine removal (RR) of syndesmotic screws regarding functional outcome.

Methods

Adult patients (aged above 17 years) with traumatic syndesmotic injury, surgically treated within 14 days of trauma using one or two syndesmotic screws, were eligible (n = 490) for inclusion in this randomized controlled noninferiority trial. A total of 197 patients were randomized for either ODR (retaining the syndesmotic screw unless there were complaints warranting removal) or RR (screw removed at eight to 12 weeks after syndesmotic fixation), of whom 152 completed the study. The primary outcome was functional outcome at 12 months after screw placement, measured by the Olerud-Molander Ankle Score (OMAS).

Introduction. The purpose of this study was to determine if better outcomes occur with use of robotic-arm assistance by comparing consecutive series of non-robotic assisted (NR-TKA) and robotic-arm assisted (NR-TKA) total knee arthroplasties with the same implant. Methods. 80 NR-TKAs and then 101 RA-TKAs were performed consecutively. 70 knees in each group that had a minimum two-year follow-up were retrospectively reviewed. Range of motion, Knee Society (KS) scores, and forgotten joint scores (FJS) were compared using Mann-Whitney U tests. Tourniquets, used for all cases, had their inflation time recorded. Component realignment to minimize soft tissue releases was used in both groups with the goal to stay within a mechanical alignment of 3° of varus to 2° of valgus. The use of soft tissue releases for balance were compared. Results. There were no statistical differences in baseline characteristics including pre-operative Knee Society scores between cohorts. The two-year NR-TKA and RA-TKA median KS knee and functional scores were 99.0 and 90.0 and 100.0 and 100.0 respectively. Mann-Whitney U test indicated a statistically significant difference in KS-KS (p<.00001) and near statistically significant difference in KS-FS (p=0.075). The 10-point higher KS-FS is considered a minimal clinically important difference. The median FJS at two years for the NR-TKA was 61.5 and the RA-TKA was 75.0. Although not statistically significant (p=0.1556), the 13.5-point increase in the RA-TKA cohort also represents a minimal clinically important difference. RA-TKA patients had statistically significant 5° higher knee flexion (p<.00001). Desired post-operative coronal alignment was present in 92.9% of NR-TKAs and 94.3% of RA-TKAs. 28.6% more of the RA-TKA cases were able to be balanced without a soft tissue release. Median tourniquet time was only 3.9 minutes longer for the robotic-arm assisted cohort. Conclusion. This comparison study demonstrates potential benefits in use of robotic-arm assistance over manual instrumentation in TKA

Aims. This study investigates the effects of intra-articular injection of adipose-derived mesenchymal stem cells (AdMSCs) and platelet-rich plasma (PRP) on lameness, pain, and quality of life in osteoarthritic canine patients. Methods. With informed owner consent, adipose tissue collected from adult dogs diagnosed with degenerative joint disease was enzymatically digested and cultured to passage 1. A small portion of cells (n = 4) surplus to clinical need were characterized using flow cytometry and tri-lineage differentiation. The impact and degree of osteoarthritis (OA) was assessed using the Liverpool Osteoarthritis in Dogs (LOAD) score, Modified Canine Osteoarthritis Staging Tool (mCOAST), kinetic gait analysis, and diagnostic imaging. Overall, 28 joints (25 dogs) were injected with autologous AdMSCs and PRP. The patients were followed up at two, four, eight, 12, and 24 weeks. Data were analyzed using two related-samples Wilcoxon signed-rank or Mann-Whitney U tests with statistical significance set at p < 0.05. Results. AdMSCs demonstrated stem cell-like characteristics. LOAD scores were significantly lower at week 4 compared with preinjection (p = 0.021). The mCOAST improved significantly after three months (p = 0.001) and six months (p = 0.001). Asymmmetry indices decreased from four weeks post-injection and remained significantly lower at six months (p = 0.025). Conclusion. These improvements in quality of life, reduction in pain on examination, and improved symmetry in dogs injected with AdMSCs and PRP support the effectiveness of this combined treatment for symptom modification in canine OA for six months. Olecranon plates used for the internal fixation of complex olecranon fractures are applied directly over the triceps tendon on the posterior aspect of the olecranon. The aim of the study is to describe the relationship of the plates and screws to the triceps tendon at the level of the olecranon. Eight cadaveric elbows were used. Dimensions of the triceps tendon at the insertion and 1cm proximal were measured. A long or a short olecranon plate was then applied over the olecranon and the most proximal screw applied. The length of the plate impinging on the tendon and the level of the screw tract on the tendon and bone were measured. The mean olecranon height was 24.3cm (22.4-26.9cm) with a tip-to-tendon distance of 14.5cm (11.9-16.2cm). The triceps tendon footprint averaged 13.3cm (11.7-14.9cm) and 8.8cm (7.6-10.2cm) in width and length, respectively. The mean width of the central tendon 1 cm proximal to the footprint was 6.8 cm. The long olecranon plate overlay over more movable tendon length than did the short plate and consequently the superior screw pierced the triceps tendon more proximally with the long plate. Using the Mann-Whitney U test, the differences were significant. The long olecranon plates encroach on more triceps tendon than short plates. This may be an important consideration for olecranon fractures with regards implant loosening or triceps tendon injury

Currently, the consensus regarding subscapularis tendon repair during a reverse total shoulder arthroplasty (rTSA) is to do so if it is possible. Repair is thought to decrease the risk of dislocation and improve internal rotation but may also increase stiffness and improvement in internal rotation may be of subclinical benefit. Aim is to retrospectively evaluate the outcomes of rTSA, with or without a subscapularis tendon repair. We completed a retrospective review of 51 participants (25 without and 26 with subscapularis repair) who received rTSR by a single-surgeon using a single-implant. Three patient reported outcome measures (PROM) were assessed pre-operatively and post-operative at twelve months, as well as range of movement (ROM) and plain radiographs. Statistical analysis utilized unpaired t tests for parametric variables and Mann-Whitney U test for nonparametric variables. External Rotation ROM pre-operatively was the only variable with a significance difference (p=0.02) with the subscapularis tendon repaired group having a greater range. Pre- and post-operative abduction (p=0.72 & 0.58), forward flexion (p=0.67 & 0.34), ASES (p=0.0.06 & 0.78), Oxford (p=0.0.27 & 0.73) and post-operative external rotation (p=0.17). Greater external rotation ROM pre-operatively may be indicative of the ability to repair the subscapularis tendon intra-operatively. However, repair does not seem to improve clinical outcome at 12 months. There was no difference of the PROMs and AROMs between the subscapularis repaired and not repaired groups for any of the variables at the pre-operative or 12 month post operative with the exception of the external rotation ROM pre-operatively. We can conclude that from PROM or AROM perspective there is no difference if the tendon is repaired or not in a rTSR and indeed the patients without the repair may have improved outcomes at 12 months

Background. Musculoskeletal disorders, including low back pain, affects 68% of UK physiotherapists across their career with patient handling considered a key risk factor. Manual handling training is mandatory for all allied health professionals, however there is limited research investigating whether professionals adopt recommended manual handling principles following training. Purpose of Study. To investigate spinal angles when facilitating sit-to-stand, and a turning manoeuvre in bed, comparing first-year physiotherapy students (who have not received manual handling training) with final-year physiotherapy students (who have received manual handling training). Methods. Cross-sectional pilot study (n= 20; 10 first-year, 10 final-year). All participants were exposed to a short training video outlining how to safely perform each manoeuvre. Retroreflective markers were attached to: L4, ASIS, PSIS, T12, C7, tragus and canthus. Spinal (neck, thoracic, lumbar and pelvic) angles were established via digital photographs using a bespoke MATLAB programme (MathWorks). A Mann-Whitney U test was conducted to determine between group differences. Results. No statistically significant results were observed between first-year and final-year students for both manoeuvres (p<0.05), except for neck angle during the turning manoeuvre (final-year students demonstrating more upright postures, p=0.037). Interestingly, for the turning manoeuvre in bed it was noted that none of the participants adjusted the bed height. Conclusion. The results suggest that clinical experience and routine manual handling training may not have a significant effect on spinal posture, especially in relation to pelvic, lumbar and thoracic angles. Further work is needed to understand how training relates to adoption of manual handling principles in practice. Conflicts of interest. None. Sources of funding. None

Background. Prospective study to compare patient reported outcome measures (PROM) for sacroiliac joint (SIJ) fusion using HA-coated screw (HACS) vs triangular titanium dowel (TTD). First study of its kind in English literature. Methods. 40 patients underwent percutaneous SIJ stabilisation using HACS and TTD was used in 70 patients. Patients were followed up closely and outcome scores were collected prospectively. PROMs were collected preoperatively and 12 months after surgery. Short Form (SF)-36, Oswestry Disability Index, EuroQol-5D-5L and Majeed Pelvic Scores were collected. Shapiro-wilk test was used to determine normality of data. Mann-whitney U test was used to compared non-parametric data and Independent sample T test for parametric data. Results. 33 patients in HACS group and 61 in TTD group completed follow-up. There was no significant difference in all preoperative PROMs in both groups hence the data was considered comparable. All postoperative PROMs were significantly higher in the TTD group. In the HACS group, 21 patients (63%) had lysis around the screw and a sub-group analysis showed that improvement in PROMs was significantly less in patients with lysis around the screw. 4 patients with lysis around the screw were offered revision due to ongoing pain. Revision was successful in only 1 patient. In TTD group only 5 patients (8.2 %) patients had radiological evidence of lysis. Relative risk of developing lysis was 6.7 times higher in HACS group. Conclusion. Percutaneous SIJ fixation procedure has been shown to have good clinical outcomes and TTD leads to significantly better patient reported outcomes compared to HACS. There is a 6.7 time higher risk of lysis with use of HACS and lysis is a risk factor for poor outcomes

Aims. Although there is increasing legalization of the use of cannabis in the USA, few well-powered studies have evaluated the association between cannabis use disorder and outcomes following primary total hip arthroplasty (THA). Thus, the aim of this study was to determine whether patients who use cannabis and undergo primary THA have higher rates of in-hospital length of stay (LOS), medical complications, implant-related complications, and costs. Methods. Using an administrative database, patients with cannabis use disorder undergoing primary THA were matched to a control group in a 1:5 ratio by age, sex, and various medical comorbidities. This yielded 23,030 patients (3,842 in the study group matched with 19,188 in the control group). The variables which were studied included LOS, 90-day medical complications, two-year implant-related complications, and 90-day costs of care. Mann-Whitney U tests were used to compare LOS and costs. Multivariate logistic regression analyses were used to calculate the odds ratios (ORs) of developing complications. Results. We found that patients in the study group had a significantly longer mean LOS compared with the controls (four days vs three days; p < 0.0001).The study group also had a significantly higher incidence and odds of developing medical (23.0 vs 9.8%, OR 1.6; p < 0.0001) and implant-related complications (16 vs 7.4%, OR 1.6; p < 0.0001) and incurred significantly higher mean 90-day costs ($16,938.00 vs $16,023.00; p < 0.0001). Conclusion. With the increasing rates of cannabis use, these findings allow orthopaedic surgeons and other healthcare professionals to counsel patients with cannabis use disorder about the possible outcomes following their THA, with increased hospital stays, complications, and costs. Aims. The optimum clearance between the bearing surfaces of hip arthroplasties is unknown. Theoretically, to minimize wear, it is understood that clearances must be low enough to maintain optimal contact pressure and fluid film lubrication, while being large enough to allow lubricant recovery and reduce contact patch size. This study aimed to identify the relationship between diametrical clearance and volumetric wear, through the analysis of retrieved components. Methods. A total of 81 metal-on-metal Pinnacle hips paired with 12/14 stems were included in this study. Geometrical analysis was performed on each component, using coordinate and roundness measuring machines. The relationship between their as-manufactured diametrical clearance and volumetric wear was investigated. The Mann-Whitney U test and unpaired t-test were used, in addition to calculating the non-parametric Spearman's correlation coefficient, to statistically evaluate the acquired data. Results. The hips in this study were found to have had a median unworn diametrical clearance of 90.31 μm (interquartile range (IQR) 77.59 to 97.40); 32% (n = 26) were found to have been below the manufacturing tolerance. There was no correlation found between clearance and bearing (r. s. = -0.0004, p = 0.997) or taper (r. s. = 0.0048, p = 0.966) wear rates. The wear performance of hips manufactured within and below these specifications was not significantly different (bearing: p = 0.395; taper: p = 0.653). Pinnacles manufactured from 2007 onwards had a greater prevalence of bearing clearance below tolerance (p = 0.004). Conclusion. The diametrical clearance of Pinnacle hips did not influence their wear performance, even when below the manufacturing tolerance. Aims. Two-stage exchange revision total hip arthroplasty (THA) performed in case of periprosthetic joint infection (PJI) has been considered for many years as being the gold standard for the treatment of chronic infection. However, over the past decade, there have been concerns about its safety and its effectiveness. The purposes of our study were to investigate our practice, collecting the overall spacer complications, and then to analyze their risk factors. Methods. We retrospectively included 125 patients with chronic hip PJI who underwent a staged THA revision performed between January 2013 and December 2019. All spacer complications were systematically collected, and risk factors were analyzed. Statistical evaluations were performed using the Student's t-test, Mann-Whitney U test, and Fisher's exact test. Results. Our staged exchange practice shows poor results, which means a 42% mechanical spacer failure rate, and a 20% recurrent infection rate over the two years average follow-up period. Moreover, we found a high rate of spacer dislocation (23%) and a low rate of spacer fracture (8%) compared to the previous literature. Our findings stress that the majority of spacer complications and failures is reflecting a population with high comorbid burden, highlighted by the American Society of Anesthesiology grade, Charlson Comorbidity Index, and Lee score associations, as well as the cardiac, pulmonary, kidney, or hepatic chronic conditions. Conclusion. Our experience of a two-stage hip exchange revision noted important complication rates associated with high failure rates of polymethylmethacrylate spacers. These findings must be interpreted in the light of the patient’s comorbidity profiles, as the elective population for staged exchange has an increasing comorbid burden leading to poor results. In order to provide better results for this specific population, our conclusion suggests that comparative strategy studies are required to improve our therapeutic indication. Aims. The purpose of our study was to determine which groups of orthopaedic providers favour virtual care, and analyze overall orthopaedic provider perceptions of virtual care. We hypothesize that providers with less clinical experience will favour virtual care, and that orthopaedic providers overall will show increased preference for virtual care during the COVID-19 pandemic and decreased preference during non-pandemic circumstances. Methods. An orthopaedic research consortium at an academic medical system developed a survey examining provider perspectives regarding orthopaedic virtual care. Survey items were scored on a 1 to 5 Likert scale (1 = “strongly disagree”, 5 = “strongly agree”) and compared using nonparametric Mann-Whitney U test. Results. Providers with less experience were more likely to recommend virtual care for follow-up visits (3.61 on the Likert scale (SD 0.95) vs 2.90 (SD 1.23); p = 0.006) and feel that virtual care was essential to patient wellbeing (3.98 (SD 0.95) vs 3.00 (SD 1.16); p < 0.001) during the pandemic. Less experienced providers also viewed virtual visits as providing a similar level of care as in-person visits (2.41 (SD 1.02) vs 1.76 (SD 0.87); p = 0.006) and more time-efficient than in-person visits (3.07 (SD 1.19) vs 2.34 (SD 1.14); p = 0.012) in non-pandemic circumstances. During the pandemic, most providers viewed virtual care as effective in providing essential care (83.6%, n = 51) and wanted to schedule patients for virtual care follow-up (82.2%, n = 50); only 10.9% (n = 8) of providers preferred virtual visits in non-pandemic circumstances. Conclusion. Orthopaedic providers with less clinical experience seem to favourably view virtual care both during the pandemic and under non-pandemic circumstances. Providers in general appear to view virtual care positively during the pandemic but are less accommodating towards it in non-pandemic circumstances. Aims. To study the associations of lumbar developmental spinal stenosis (DSS) with low back pain (LBP), radicular leg pain, and disability. Methods. This was a cross-sectional study of 2,206 subjects along with L1-S1 axial and sagittal MRI. Clinical and radiological information regarding their demographics, workload, smoking habits, anteroposterior (AP) vertebral canal diameter, spondylolisthesis, and MRI changes were evaluated. Mann-Whitney U tests and chi-squared tests were conducted to search for differences between subjects with and without DSS. Associations of LBP and radicular pain reported within one month (30 days) and one year (365 days) of the MRI, with clinical and radiological information, were also investigated by utilizing univariate and multivariate logistic regressions. Results. Subjects with DSS had higher prevalence of radicular leg pain, more pain-related disability, and lower quality of life (all p < 0.05). Subjects with DSS had 1.5 (95% confidence interval (CI) 1.0 to 2.1; p = 0.027) and 1.8 (95% CI 1.3 to 2.6; p = 0.001) times higher odds of having radicular leg pain in the past month and the past year, respectively. However, DSS was not associated with LBP. Although, subjects with a spondylolisthesis had 1.7 (95% CI 1.1 to 2.5; p = 0.011) and 2.0 (95% CI 1.2 to 3.2; p = 0.008) times greater odds to experience LBP in the past month and the past year, respectively. Conclusion. This large-scale study identified DSS as a risk factor of acute and chronic radicular leg pain. DSS was seen in 6.9% of the study cohort and these patients had narrower spinal canals. Subjects with DSS had earlier onset of symptoms, more severe radicular leg pain, which lasted for longer and were more likely to have worse disability and poorer quality of life. In these patients there is an increased likelihood of nerve root compression due to a pre-existing narrowed canal, which is important when planning surgery as patients are likely to require multi-level decompression surgery. Aims. It remains difficult to diagnose early postoperative periprosthetic joint infection (PJI) following total knee arthroplasty (TKA). We aimed to validate the optimal cutoff values of ESR, CRP, and synovial fluid analysis for detecting early postoperative PJI in a large series of primary TKAs. Methods. We retrospectively identified 27,066 primary TKAs performed between 2000 and 2019. Within 12 weeks, 169 patients (170 TKAs) had an aspiration. The patients were divided into two groups: those evaluated ≤ six weeks, or between six and 12 weeks postoperatively. The 2011 Musculoskeletal Infection Society (MSIS) criteria for PJI diagnosis in 22 TKAs. The mean follow-up was five years (two months to 17 years). The results were compared using medians and Mann-Whitney U tests and thresholds were analyzed using receiver operator characteristic curves. Results. Within six weeks, the median CRP (101 mg/l vs 35 mg/l; p = 0.011), synovial WBCs (58,295 cells/μl vs 2,121 cells/μl; p ≤ 0.001), percentage of synovial neutrophils (91% vs 71% (p < 0.001), and absolute synovial neutrophil count (ANC) (50,748 cells/μl vs 1,386 cells/μl (p < 0.001) were significantly higher in infected TKAs. Between six and 12 weeks, the median CRP (85 mg/l vs 5 mg/l (p < 0.001)), ESR (33 mm/hr vs 14 mm/hr (p = 0.015)), synovial WBCs (62,247 cells/μl vs 620 cells/μl (p < 0.001)), percentage of synovial neutrophils (93% vs 54% (p < 0.001)), and ANC (55,911 cells/μl vs 326 cells/μl (p < 0.001)) were also significantly higher in infected TKAs. Optimal thresholds at ≤ six weeks were: CRP ≥ 82 mg/l (sensitivity 70%, specificity 77%), synovial WBCs ≥ 8,676 cells/μl (83%, 90%), percentage of synovial neutrophils ≥ 88% (67%, 78%), and ANC ≥ 8,346 cells/μl (83%, 91%). Between six and 12 weeks, thresholds were: CRP ≥ 34 mg/l (90%, 93%), synovial WBCs ≥ 1,983 cells/μl (80%, 85%), percentage of synovial neutrophils ≥ 76% (80%, 81%), and ANC ≥ 1,684 cells/μl (80%, 87%). Conclusion. Early PJI after TKA should be suspected within six weeks if the CRP is ≥ 82 mg/l, synovial WBCs are ≥ 8,676 cells/μl, the percentage of synovial neutrophils is ≥ 88%, and/or the ANC is ≥ 8,346 cells/μl. Between six and 12 weeks, thresholds include a CRP of ≥ 34 mg/l, synovial WBC of ≥ 1,983 cells/μl, a percentage of synovial neutrophils of ≥ 76%, and/or an ANC of ≥ 1,684 cells/μl. Aims. Idiopathic scoliosis is the most common spinal deformity in adolescents and children. The aetiology of the disease remains unknown. Previous studies have shown a lower bone mineral density in individuals with idiopathic scoliosis, which may contribute to the causation. The aim of the present study was to compare bone health in adolescents with idiopathic scoliosis with controls. Methods. We included 78 adolescents with idiopathic scoliosis (57 female patients) at a mean age of 13.7 years (8.5 to 19.6) and 52 age- and sex-matched healthy controls (39 female patients) at a mean age of 13.8 years (9.1 to 17.6). Mean skeletal age, estimated according to the Tanner-Whitehouse 3 system (TW3), was 13.4 years (7.4 to 17.8) for those with idiopathic scoliosis, and 13.1 years (7.4 to 16.5) for the controls. Mean Cobb angle for those with idiopathic scoliosis was 29° (SD 11°). All individuals were scanned with dual energy x-ray absorptiometry (DXA) and peripheral quantitative CT (pQCT) of the left radius and tibia to assess bone density. Statistical analyses were performed with independent-samples t-test, the Mann-Whitney U test, and the chi-squared test. Results. Compared with controls, adolescents with idiopathic scoliosis had mean lower DXA values in the left femoral neck (0.94 g/cm. 2. (SD 0.14) vs 1.00 g/cm. 2. (SD 0.15)), left total hip (0.94 g/cm. 2. (SD 0.14) vs 1.01 g/cm. 2. (SD 0.17)), L1 to L4 (0.99 g/cm. 2. (SD 0.15) vs 1.06 g/cm. 2. (SD 0.17)) and distal radius (0.35 g/cm. 2. (SD 0.07) vs 0.39 g/cm. 2. (SD 0.08; all p ≤ 0.024), but not in the mid-radius (0.72 g/cm. 2. vs 0.74 g/cm. 2. ; p = 0.198, independent t-test) and total body less head (1,559 g (SD 380) vs 1,649 g (SD 492; p = 0.0.247, independent t-test). Compared with controls, adolescents with idiopathic scoliosis had lower trabecular volume bone mineral density (BMD) on pQCT in the distal radius (184.7 mg/cm. 3. (SD 40.0) vs 201.7 mg/cm. 3. (SD 46.8); p = 0.029), but not in other parts of the radius or the tibia (p ≥ 0.062, Mann-Whitney U test). Conclusion. In the present study, idiopathic scoliosis patients seemed to have lower BMD at central skeletal sites and less evident differences at peripheral skeletal sites when compared with controls. Abstract. Objectives. Exploring the relationship of gait function pre and post total knee replacement (TKR) in two groups of patients. Methods. Three-dimensional gait analysis was performed at Cardiff University, UK, and Karolinska University Hospital, Sweden, on 29 and 25 non-pathological (NP) volunteers, and 39 and 28 patients with end-stage knee osteoarthritis (OA), respectively. Patients were assessed pre and one-year post-TKR. Data reduction was performed via Principal Component (PC) analysis on twenty-four kinematic and kinetic waveforms in both NP and pre/post-TKR. Cardiff's and Karolinska's cohorts were analysed separately. The Cardiff Classifier, a classification system based on the Dempster-Shafer theory, was trained with the first 3 PCs of each variable for each cohort. The Classifier classifies each participant by assigning them a belief in NP, belief in OA (BOA) and belief in uncertainty, based on their biomechanical features. The correlation between patient's BOA values (range: 0–1, 0 indicates null BOA and 1 high BOA) pre and post-TKR was tested through Spearman's correlation coefficient in each cohort. The related-samples Wilcoxon signed-rank test (α=0.05) determined the significant changes in BOA in each cohort of patients. The Mann-Whitney U test (α=0.05) was run to explore differences between the patients’ cohorts. Results. There were no significant differences between patients’ cohorts in median age (p=0.096), height (p=0.673), weight (p=0.064) or KOOS sub-scores pre or post-TKR (p-value ranged 0.069 to 0.955) but Cardiff's patients had a significantly higher BMI (p=0.047). There was a significant, median decrease of 0.12 and 0.19 in the BOA pre to post TKR (p<0.001) in Cardiff's and Karolinska's patients, respectively. There was a statistically significant, strong positive correlation between the BOA pre and post-TKR (Cardiff:r. s. =0.706, p<0.001; Karolinska:r. s. =0.669, p<0.001). Conclusions. In two distinct cohorts of patients, having a more compromised gait function in end-stage knee OA was correlated with poorer gait function post-TKR

Background. Partial facetectomies with pedicle screw instrumentation is widespread and a well described technique for achieving posterior correction of scoliosis. Newton et al. first described the use of the UBS in the posterior correction of AIS in 2014. The aim of this study was to compare the effectiveness of the UBSPO in achieving posterior correction in Type1 AIS as compared to the traditional partial facetectomies. Aim of this study was to assess the effectiveness of USBPO in achieving posterior correction in Type 1 AIS as compared to partial facetectomies. Methods. A retrospective review of 40 patients with type 1 AIS who had undergone a posterior correction of scoliosis between 2010 and 2016 was performed. Group A (n=20) consisted of consecutive patients that had partial facetectomies while Group B (n=20) consisted of consecutive patients having UBSPO. Both groups were matched for demographic parameters. Pre and post-operative radiographic parameters and operative data in both groups were compared. The Mann-Whitney U test was used for statistical analysis. Results. There was no significant difference between the two groups in terms of age, sex, magnitude of curves, apical rotation and flexibility on the preop imaging. There was a significant difference between the mean postop Cobb angle (21.9° vs 9.8°, p<0.0005), correction (63.04% vs 84.3%, p<0.0005) and postop apical rotation (p = 0.008) in favour of the UBSPO group. At 2-year follow-up there was a statistically significant increase in the cobb angle in the facetectomy group (21.89° (immediate post op) Vs 24.64° P=0.033) and no such difference in the UBSPO group. There was no significant difference between surgical time (p = 0.536) and blood loss (p = 0.380). Conclusion. The use of the UBSPO for posterior release provides more effective correction in the coronal and axial planes than traditional partial facetectomies in type 1 AIS


There is a strong association between the presence of a calcar collar on a cementless stem and a reduced risk of revision surgery for periprosthetic fracture of the femur (PFF). A medial calcar collar may act to reduce relative movement between the implant and femur during PFF, through calcar-collar contact (CCC). The aims were:. Estimate the effect of CCC on periprosthetic fracture mechanics. Estimate the effect of initial calcar-collar separation on the likelihood of CCC. Three groups of six composite femurs were implanted with a fully coated collared cementless femoral stem. Neck resection differed between groups (group 1 = no additional resection, group 2 = 3mm additional resection, group 3 = 6mm additional resection). PFF were simulated using a previously published technique. Fracture torque and rotational displacement were measured and torsional stiffness and rotational work prior to fracture were estimated. Results between trials where CCC did and did not occur where compared using Mann-Whitney U tests. Logistic regression estimated the odds (OR) of failing with 95% confidence interval (CI) to achieve CCC for a given initial separation. Where CCC occurred fracture torque was greater (47.33 [41.03 to 50.45] Nm versus 38.26 [33.70 to 43.60] Nm, p= 0.05) and torsional stiffness was greater (151.38 [123.04 to 160.42] rad.Nm. −1. versus 96.86 [84.65 to 112.98] rad.Nm. −1. , p <0.01). CCC was occurred in all cases in group one, 50% in group two and 0% in group three. OR of failure to obtain CCC increased 3.8 fold (95% CI 1.6 to 30.2, p <0.05) for each millimetre of separation. Resistance to fracture and construct stiffness increased when a the collar made contact with the calcar prior to fracture and the chances of contact decrease with increasing initial separation at the time of implantation. Surgeons should aim to achieve a calcar-collar distance of less than 1mm following implantation to ensure CCC and to reduce the risk of fracture

Introduction and Objective. Current cartilage repair strategies lack adequate tissue integration capacity and often present mechanical failure at the graft-to-host tissue junction. The design of multilayered osteochondral tissue engineering (TE) constructs is an attractive approach to overcome these problems. However, calcium ion-release from resorbable bone-replacement materials was suggested to compromise chondrogenic differentiation of adjacent cartilage tissue and it is unclear whether articular chondrocytes (AC) or mesenchymal stroma cells (MSC) are more sensitive to such conditions. Aim of the study was to compare how elevated calcium levels affect cartilage matrix production during re-differentiation of AC versus chondrogenic differentiation of MSC. The results of this study will help to identify the ideal cell source for growth of neocartilage adjacent to a calcified bone replacement material for design of multilayered osteochondral TE approaches. Materials and Methods. Expanded human AC and MSC (6–12 donors per group) were seeded in collagen type I/III scaffolds and cultured under standard chondrogenic conditions at control (1.8mM) or elevated (8.0mM) CaCl2 for 35 days. Proteoglycan and collagen production were assessed via radiolabel-incorporation, ELISA, qPCR and Western blotting. Differences between groups or cell types were calculated using the non-parametric Wilcoxon or Mann-Whitney U test, respectively, with p < 0.05 considered significant. Results. Elevated calcium significantly reduced GAG synthesis (63% of control, p=0.04) and chondrogenic marker expression of AC, lowering the GAG/DNA content (47% of control, p=0.004) and collagen type II deposition (24% of control, p=0.05) of neocartilage compared to control conditions. Opposite, at elevated calcium levels MSC-derived chondrocytes significantly increased GAG synthesis (130% of control, p=0.02) and collagen type II content (160% of control, p=0.03) of cartilage compared to control tissue. Chondrogenic and hypertrophic marker expression was insensitive to calcium levels in MSC-derived chondrocytes. As a result, maturation under elevated calcium allowed for a significantly higher GAG/DNA content in MSC-derived samples compared to AC constructs, although under control conditions both groups developed similarly. Conclusions. AC and MSC showed an opposite reaction to elevation of calcium levels regarding cartilage matrix production and we propose MSC as a preferred cell source to grow chondrocytes in vicinity to calcified bone replacement materials. Since MSC remained prone to hypertrophy under elevated calcium, trizonal cartilage TE constructs, where an AC-layer is separated from the bone replacement phase by an intermediate layer of MSC appear as an ideal design for multilayered osteochondral TE with respect to calcium sensitivity of cells and protection of the upper cartilage layer from hypertrophy

Purpose. The purpose of this study was to prospectively evaluate the rehabilitation outcome of children following operative and non-operative stabilisation of long bone fractures sustained in conjunction with an acquired brain injury (ABI). Materials and Methods. Between 1996 and 2002, children up to 16 years of age who were admitted to the Paediatric Intensive Care Unit (PICU) with an ABI and concomitant tibial or femoral shaft fractures were considered eligible for inclusion. Children who died or were unable to walk before the accident were excluded. All data relating to the sustained injuries, the duration of PICU inpatient stay, the fracture treatment and the functional outcome were collected prospectively. The severity of the injuries was assessed using the Injury Severity Score (ISS) and the Glasgow Coma Score (GCS). The duration of time was taken from admission to reaching rehabilitation milestones; the ability to sit, stand and walk was then assessed. Total duration of hospital inpatient stay and mobility on discharge were also recorded. Operative skeletal stabilisation included external or internal fixation, as well as flexible intramedullary nailing. Statistical analysis was performed using the Mann-Whitney U Test. Results. From 300 children admitted to the PICU for treatment of ABI, thirty-seven fulfilled the criteria for inclusion into the study. For both groups (operative vs nonoperative treatment) the mean age of patients was 9 years (range 1–15yrs) and the ISS, GCS scores and the mean stay in PICU were similar. There was a total of 21 femoral fractures (11 treated conservatively and 10 operatively) and 16 tibial fractures (7 conservatively and 9 operatively). Overall, the results showed a reduction in the mean time taken to reach all rehabilitation milestones following operative fracture stabilisation when compared to those treated non-operatively. The mean time to sitting was reduced from 27.1 days to 17.8. The mean time to standing was reduced from 50.1 to 35.6 and to walking, from 67.1 to 45.7 days. The Mann-Whitney U Test demonstrated a statistically significant reduced time to walk in the operatively treated group (p<0.05). Due to the small size of the sample group, significant statistical data for the other parameters measured was not evident. Conclusion. This study was specifically aimed at the short to medium term gains of operative treatment of tibial and femoral fractures in association with ABI. It has been shown that a co-ordinated rehabilitation programme plus operative stabilisation of children's tibial and femoral shaft fractures sustained in association with ABI, hastens rehabilitation, as it allowed aggressive rehabilitation programme to commence at a much earlier stage without risk of loss of fracture reduction

Targeted delivery of drugs is a major challenge in diseases such as infections and tumors. The aim of this study was to demonstrate that hydroxyapatite (HA) particles can act as a recruiting moiety for various bioactive molecules and as a proof-of-concept demonstrate that the affinity of drugs to hydroxyapatite can exert a biological effect. A bisphosphonate, zoledronic acid (ZA), was used as a model drug. Experiment 1 (ZA seeks HA): Calcium sulphate (CaS)/hydroxyapatite (HA) biomaterial pellets (diameter¸=5 mm, height=2 mm) were implanted in the abdominal muscle pouch of rats. After 2-weeks of implantation, a sub-cutaneous injection of 14C-ZA (0.1 mg/kg) was given. 24 h later, the animals were sacrificed and the uptake of ZA determined in the pellets using scintillation counting. Experiment 2 (Systemically administered ZA seeks HA and exerts a biological effect): A fenestrated implant was filled with the CaS/HA biomaterial and inserted in the proximal tibia of rats. 2-weeks post-op, a subcutaneous injection of ZA (0.1 mg/kg) was given. Animals were sacrificed at 6-weeks post-op. Empty implant was used as a control. Peri-implant bone formation was evaluated using different techniques such as micro-CT, mechanical testing and histology. Welch's t-test was used for mechanical testing and Mann-Whitney U test for micro-CT data analysis. Experiment 1: Uptake of radioactive ZA in the CaS/HA biomaterial was confirmed. Almost no ZA was present in the surrounding muscle. These results show high specific binding between systemically administered ZA and synthetic particulate HA. Experiment 2: Significantly higher peri-implant bone was measured using micro-CT in the group wherein the implant contained the CaS/HA biomaterial and ZA was administered systemically (This study presents a method for biomodulating HA in situ by different bioactive molecules. The approach of implanting a biomaterial capable of recruiting systemically given drugs and thereby activate the material is novel and may present a possibility to treat bone infections or tumors

Abstract. Objective. To estimate the effect of calcar collar contact on periprosthetic fracture mechanics using a collared fully coated cementless femoral stem. Methods. Three groups of six composite femurs were implanted with a fully coated collared cementless femoral stem. Neck resection was increased between groups (group 1 = normal, group 2 = 3mm additional, group 3 = 6mm additional), to simulate failure to obtain calcar collar contact. Periprosthetic fractures of the femur were simulated using a previously published technique. Fracture torque and rotational displacement were measured and torsional stiffness and rotational work prior to fracture were estimated. High speed video recording identified if collar to calcar contact (CCC) occurred. Results between trials where calcar contact did and did not occur where compared using Mann-Whitney U tests. Results. Where CCC occurred versus where no CCC occurred, fracture torque was greater (47.33 [41.03 to 50.45] Nm versus 38.26 [33.70 to 43.60] Nm, p= 0.05), Rotational displacement was less (0.29 [0.27 to 0.39] rad versus 0.37 [0.33 to 0.49] rad, p= 0.07), torsional stiffness was greater (151.38 [123.04 to 160.42] rad. Nm-1 versus 96.86 [84.65 to 112.98] rad.Nm-1, p <0.01) and rotational work was similar (5.88 [4.67, 6.90] J versus 5.31 [4.40, 6.56] J, p= 0.6). Conclusions. Resistance to fracture and construct stiffness increased when a collared cementless stem made contact with the femoral calcar prior to fracture. These results demonstrate that calcar-collar contact and not a calcar collar per se, is crucial to maximising the protective effect of a medial calcar collar on the risk of post-operative periprosthetic fractures of the femur. We report a cost-effectiveness analysis of Primary Total Ankle Replacements (TAR). In addition, we looked for factors that could be predictors of increased gain in Quality-Adjusted Life Years (QALYs). Pre-operative and six-month post-operative data for TAR was obtained over seven-and-a-half-years in NHS Lothian. The EuroQol general health questionnaire (EQ-5D-3L) measured health-related Quality of Life and the Manchester-Oxford Foot Questionnaire (MOXFQ) measured joint function. Predictors, tested for significance with QALYs gained, were pre-operative scores and demographic data including age, gender, BMI and Deprivation category. Continuous variables were assessed with Pearson correlation coefficients, Deprivation and BMI categories with Boxplots and gender with the Mann-Whitney U test. The 74-patient cohort [Mean age 68.03 (SD 8.61), 60.8% male] had 49% classed as obese or higher. Cost per QALY gained was £3841, rising to £9202 when annual 3.5% reduction in health gains and 1.9% revision rates were included. Lower pre-operative EQ-5D-3L index correlated significantly with increased QALYs gained (p <0.01), all other predictors returned insignificant results (p >0.05). Primary TAR has proven very cost-effective in treating end-stage ankle arthritis. Pre-operative EQ-5D-3L is the most promising predictor of QALYs gained identified

Staphylococcus aureus is the main cause of osteomyelitis and forms biofilm and staphylococcal abscess communities (SACs) in humans. While S. aureus has several toxins with specificity for human targets and working with human host cells would be preferred, for SACs no in vitro models, two-dimensional (2D) or three-dimensional (3D), have been described in literature to date. Advanced 3D in vitro cell culture models enable the incorporation of human cells and resemble in vivo tissue more closely than conventional 2D cell culture. Therefore, the aim of this study was to develop an in vitro model of SACs by using a 3D system. The model should allow for studies into antibiotic tolerance and S. aureus - human host cells interactions. With a clinical isolate (S. aureus JAR) or a lab strain (S. aureus ATCC 49230-GFP), SACs were grown in a collagen gel (1.78 mg/ml, Gibco) supplemented with 200 µl human plasma at 37 °C. Transmission and scanning electron microscopy was used to obtain a detailed overview of SACs, whereas immunofluorescent stainings were done to determine whether the pseudocapsule around SACs consist of fibrin. Antibiotic tolerance of SACs was assessed with 100× the minimal inhibitory concentration (MIC) of gentamicin (Roth). Bacterial clearance of non-establised SACs and established SACs with or without pseudocapsule was determined by exposure to differentiated PLB neutrophil-like cells (differentiation with 1.25% DMSO and 5% FBS for 5 days; dPLB) or primary neutrophils isolated with lymphoprep from fresh heparin blood. Degradation of the pseudocapsule was done with 7.5 µl/ml plasmin (Sigma). Colony forming unit (CFU) counts were performed as quantification method. Statistical analysis was performed with the ANOVA multiple comparison test or, when data was not normally distributed, with a Mann-Whitney U test. We have developed a 3D in vitro model of SACs which after overnight growth were on average 200 micrometers in diameter, consisted of 8 log10 CFUs and were surrounded by an inner and outer fibrin pseudocapsule. The in vitro grown SACs tolerated 100× the MIC of gentamicin for 24h and did not significantly differ from control SACs (p=0.1000). dPLB neutrophil-like cells or primary neutrophils did not clear established in vitro SACs (p=0.1102 and p=0.8767, respectively). When the fibrin pseudocapsule was degraded by the enzyme plasmin, dPLB neutrophil-like cells or primary neutrophils caused for a significant decrease in total CFU compared the SACs that did had a pseudocapsule (p=0.0333 and p=0.0272, respectively). The in vitro SACs model offers a tool for host-pathogen interaction and drug efficacy assessments and is a valuable starting point for future research

Syndesmotic ankle injuries are present in one fourth of all ankle trauma and may lead to chronic syndesmotic instability as well as posttraumatic ankle osteoarthritis. The main challenge remains distinguishing them from other types of ankle trauma. Currently, the patient's injured and non-injured ankles are compared using plain radiographs to determine pathology. However, these try to quantify 3D displacement using 2D measurements techniques and it is unknown to what extent the 3D configuration of the normal ankle syndesmosis is symmetrical. We aimed to assess the 3D symmetry of the normal ankle syndesmosis between the right and left side in a non- and weightbearing CT. In this retrospective comparative cohort study, patients with a bilateral non-weightbearing CT (NWBCT; N=28; Mean age=44, SD=17.4) and weight-bearing CT (WBCT; N=33; Mean age=48 years; SD=16.3) were analyzed. Consecutive patients were included between January 2016 and December 2018 when having a bilateral non-weightbearing or weightbearing CT of the foot and ankle. Exclusion criteria were the presence of hindfoot pathology and age less than 18 years or greather than 75 years. CT images were segmented to obtain 3D models. Computer Aided Design (CAD) operations were used to fit the left ankle on top of the right ankle. The outermost point of the apex of the lateral malleolus (AML), anterior tubercle (ATF) and posterior tubercle (PTF) were computed. The difference in the coordinates attached to these anatomical landmarks of the left distal fibula in the ankle syndesmosis with respect to right were used to quantify symmetry. A Cartesian coordinate system was defined based on the tibia to obtain the direction of differences in all six degrees of freedom. Statistical analysis was performed using the Mann-Whitney U test to allow comparison between measurements from a NWBCT and WBCT. Reference values were determined for each 3D measurement in a NWBCT and WBCT based on their 2SD. The highest difference in translation could be detected in the anterior-posterior direction (Mean AP. NWBCT. = −0.01mm; 2SD=3.43/Mean AP. WBCT. =−0.1mm; 2SD=2.3) and amongst rotations in the external direction (Mean AP. NWBCT. =−0.3°; 2SD=6.7/Mean AP. WBCT. =-0,2°; 2SD=5.2). None of these differences were statistically significant in the normal ankle syndesmosis when obtained from a NWBCT compared to a WBCT (P>0.05). This study provides references values concerning the 3D symmetry of the normal ankle syndesmosis in weightbearing and non-weightbearing CT-scans. These novel data contribute relevantly to previous 2D radiographic quantifications. In clinical practice they will aid in distinguishing if a patient with a syndesmotic ankle lesion differs from normal variance in syndesmotic ankle symmetry

Introduction. The application of artificial intelligence (A.I) using patient reported outcomes (PROs) to predict benefits, risks, benefits and likelihood of improvement following surgery presents a new frontier in shared decision-making. The purpose of this study was to assess the impact of an A.I-enabled decision aid versus patient education alone on decision quality in patients with knee OA considering total knee replacement (TKR). Secondarily we assess impact on shared decision-making, patient satisfaction, functional outcomes, consultation time, TKR rates and treatment concordance. Methods. We performed a randomized controlled trial involving 130 new adult patients with OA-related knee pain. Patients were randomized to receive the decision aid (intervention group, n=65) or educational material only (control group, n=65) along with usual care. Both cohorts completed patient surveys including PROs at baseline and between 6–12 weeks following initial evaluation or TKR. Statistical analysis included linear mixed effect models, Mann-Whitney U tests to assess for differences between groups and Fisher's exact test to evaluate variations in surgical rates and treatment concordance. Results. The intervention group showed greater decision quality (K-DQI, Mean difference = 20%, p<0.0001), collaboration in decision-making (CollaboRATE, 12% (intervention group), 47% (control group) below median, p<0.0001), satisfaction with consultations (NRS-C, 14% (intervention group), 33% (control group) below median, p=0.008), improvement in functional outcomes from baseline up to 12 week follow-up (KOOSJR, 4.9 pts higher (intervention group), p=0.029) without significantly impacting consultation time. No differences were observed in TKR rates or treatment concordance. Conclusion. A.I-enabled decision aids incorporating PROs in predictive algorithms can improve decision quality, level of shared decision-making, satisfaction with patient-provider consultations, and functional outcomes, without extending consultation times. The combination of advanced predictive technologies and patient reported data to forecast surgical outcomes presents a paradigm shift in shared decision making and the delivery of high value care for patients with knee OA

Surgical treatment options for Femoroacetabular impingement (FAI) includes both surgical dislocation and hip arthroscopy techniques. The primary aim of this study was to evaluate and compare the survivorship of arthroscopies (scope) and surgical dislocations (SD) at minimum 5-year follow-up. The secondary aim was to describe differences in functional outcomes between the 2 groups. This was a retrospective, single surgeon, consecutive, case-series from a large tertiary care centre. We evaluated all surgeries that were performed between 2005 and 2011. Our institutional database was queried for any patient undergoing surgery for FAI (pincer (n=23), cam (n=306), or mixed (n=103) types). Patients with childhood pathologies i.e. Legg Calve Perthes and slipped capital femoral epiphysis were excluded. This resulted in 221 hips (169 males, 52 females) who underwent either SD (94, 42.5%) or scope (127, 57.5%). A manual chart review was completed to identify patients who sustained a complication, underwent revision surgery or progressed to a total hip arthroplasty (THA). In addition, we reviewed prospectively collected patient reported outcome measure (PROMs) using (SF12, HOOS, and UCLA). Survivorship outcome was described for the whole cohort and compared between the 2 surgical groups. PROMs between groups were compared using The Mann-Whitney U test and the survival between groups was assessed using the Kaplan-Meier Analysis and the Log-Rank Mantel Cox test. All analyses were performed in SPSS (IBM, v. 26.0). The cohort included 110 SDs and 320 arthroscopies. The mean age of the whole cohort was 34±10; patients in the SD group (32±9) were younger compared to the arthroscopy group (39±10) (p<0.0001). There were 16 post-operative complications (similar between groups) and 77 re-operations (more common in the SD group (n=49) due to symptomatic metal work (n=34)). The overall 10-year survival was 91±3%. Survivorship was superior in the arthroscopy group at both 5- (96% (95%CI: 93 – 100)) and 8- years 94% (95%CI: 90 – 99%) compared to the SD Group (5-yr: 90% (95%CI: 83 – 98); 8-yr: 84% (95%CI:75 – 93)) (p=0.003) (Figure 1). On average HOOS improved from 54±19 to 68±22 and WOMAC from 65±22 to 75±22. The improvement in PROMs were similar between the 2 groups. We report very good long-term joint preservation for the treatment of FAI, which is similar to those reported in hip dysplasia. In addition, we report satisfactory improvement in function following such treatment. The differences reported in joint survival likely reflect selection biases from the treating surgeon; more complex cases and those associated with more complex anatomy were more likely to have been offered a SD in order to address the pathology with greater ease and hence the inferior joint preservation identified in this group. Introduction. Diagnosing acute postoperative periprosthetic joint infections (PJIs) after primary total knee arthroplasties (TKAs) remains difficult. Published diagnostic thresholds for ESR, CRP, and synovial fluid analysis have not been replicated. We aimed to validate the optimal cutoffs for detecting acute postoperative PJIs in a large series of primary TKAs. Methods. We retrospectively identified 27,066 primary TKAs performed between 2000–2019. Within 12 weeks, 171 knees (170 patients) had a synovial fluid aspiration. Patients were divided into two groups: evaluation <6 weeks or 6–12 weeks. The 2011 MSIS criteria for PJI diagnosed infection in 22 knees. Mann-Whitney U tests compared medians; ROC analyses determined optimal thresholds. Mean follow-up was 5 years. Results. Within 6 weeks, CRP (101 mg/L vs. 35 mg/L; p=0.01), synovial WBCs (58,295 cells/μL vs. 2121 cells/μL; p=<0.001), and synovial neutrophils (91% vs. 71%; p<0.001) were significantly higher in infected TKAs. ESR did not differ between groups at <6 weeks. Between 6–12 weeks, CRP (85 mg/L vs. 6 mg/L; p<0.001), ESR (33 mm/h vs. 15 mm/h; p=0.02), synovial WBCs (62,247 cells/μL vs. 596 cells/μL; p<0.001), and synovial neutrophils (93% v. 54%; p<0.001) were significantly higher in infected TKAs. Optimal thresholds at <6 weeks were: CRP ≥81 mg/L (sensitivity 80%, specificity 77%), synovial WBCs ≥8515 cells/μL (sensitivity 92%, specificity 90%), and synovial neutrophils≥86% (sensitivity 92%, specificity 72%). Between 6–12 weeks, thresholds were: CRP ≥32 mg/L (sensitivity 100%, specificity 93%), synovial WBCs ≥7450 cells/μL (sensitivity 80%, specificity 98%), and synovial neutrophils ≥84% (sensitivity 80%, specificity 89%). Conclusion. Acute PJI after TKA should be suspected within 6 weeks if CRP is ≥81 mg/L, synovial WBCs are ≥8500 cells/μL, and/or synovial neutrophils≥86%. Between 6–12 weeks, concerning thresholds include a CRP ≥ 32 mg/L, synovial WBC ≥7450, and synovial neutrophils ≥ 84%

Introduction. Neither a surgeon's intraoperative impression or computer navigation parameters have been shown to be predictive of postoperative outcomes following TKA. The purpose of this study is to determine 1) whether a surgeon and a robot can predict the 1-year KOOS pain score (KPS) and 2) determine what factors correlate with poor KOOS scores in well aligned and balanced TKA. Methods. The data of 131 consecutive patients enrolled in a prospective trial was reviewed. All TKAs were performed using a dynamic ligament tensioning robotic system with a tibial first resection technique and a cruciate sacrificing ultracongruent knee implant. Each TKA was graded based on the final recorded mediolateral ligament balance at 10° and 90°: A) <1mm with an implanted insert thickness equal to planned (n=74); B) <1mm (n=25); C) <2mm (n=26); D) >2mm (n=6) (Table-1). The 1-year KPS for each knee grade were compared and the likelihood of achieving an KPS > 90 was calculated. Finally, the factors associated with lower KPS despite achieving a high grade TKA (A/B) was performed. The Mann-Whitney U test and Chi-squared analysis was performed. Results. Patients with a grade of A and B had higher 1-year KPS compared to knees rated C and D (p=0.031) (Fig-1). There was no difference in KPS in TKAs rated A or B, but 33% in this group did not report a KPS > 90. While there was no correlation with age, sex, preoperative deformity, and preoperative KOOS and PROMIS physical scores, patients with KPS < 90 despite a TKA rated A or B had lower PROMIS metal health scores compared to patients reporting KPS > 90 (54.1 vs. 50.8, p= 0.043). Finally, Grade A and B patients who scored KPS > 90 were more likely to respond with “my expectations were too low”, and they are performing better than expected compared to Grade A and B patients who scored KPS < 90 (40% vs 22%, p = 0.004). Summary. A robotic balanced knee is correlated with higher KPS at 1 year but not predictive. Despite accurate alignment, rotation, and ligament balance information, a robotic system could not guarantee excellent pain relief. Objectives. Whilst gait speed is variable between healthy and injured adults, the extent to which speed alone alters the 3D in vivo knee kinematics has not been fully described. The purpose of this prospective study was to understand better the spatiotemporal and 3D knee kinematic changes induced by slow compared with normal self-selected walking speeds within young healthy adults. Methods. A total of 26 men and 25 women (18 to 35 years old) participated in this study. Participants walked on a treadmill with the KneeKG system at a slow imposed speed (2 km/hr) for three trials, then at a self-selected comfortable walking speed for another three trials. Paired t-tests, Wilcoxon signed-rank tests, Mann-Whitney U tests and Spearman’s rank correlation coefficients were conducted using Stata/IC 14 to compare kinematics of slow versus self-selected walking speed. Results. Both cadence and step length were reduced during slow gait compared with normal gait. Slow walking reduced flexion during standing (10.6° compared with 13.7°; p < 0.0001), and flexion range of movement (ROM) (53.1° compared with 57.3°; p < 0.0001). Slow walking also induced less adduction ROM (8.3° compared with 10.0°; p < 0.0001), rotation ROM (11.4. °. compared with 13.6. °. ; p < 0.0001), and anteroposterior translation ROM (8.5 mm compared with 10.1 mm; p < 0.0001). Conclusion. The reduced spatiotemporal measures, reduced flexion during stance, and knee ROM in all planes induced by slow walking demonstrate a stiff knee gait, similar to that previously demonstrated in osteoarthritis. Further research is required to determine if these characteristics induced in healthy knees by slow walking provide a valid model of osteoarthritic gait. Cite this article: N. Mannering, T. Young, T. The primary aim of this study was to identify the rate of osteoarthritis in scaphoid fracture non-union. We also aimed to investigate whether the incidence of osteoarthritis correlates with the duration of non-union(interval), and to identify the variables that influence the outcome. We retrospectively reviewed 273 scaphoid fracture non-union presented between 2007 and 2016. Data included patient demographics, interval, fracture morphology, grade of osteoarthritis (Kellgren-Lawrence) and scaphoid non-union advanced collapse (SNAC), and overall health-related quality of life. Patients were divided into two groups (SNAC and Non-SNAC). Group differences were analysed using Mann-Whitney U test and association with Pearson's correlations. A two-sided p-value of <0.05 was considered significant. The scaphoid fracture non-union were confirmed on CT scans (n=243) and plain radiographs (n=35). The subjects were 32 females and 260 males with the mean age of 33.8 years (SD, 13.2). The average interval was 3.1 years (range, 0–45 years). Osteoarthritis occurred in 58% (n=161) of non-unions, and 42% (n=117) had no osteoarthritis. In overall, 38.5% (n=107) had SNAC-1, 9% (n=25) with SNAC-2, and 10.4% (n=29) presented with SNAC-3. The mean interval in the non-SNAC group was 1.2 years, and in SNAC 1,2, and 3 were 2.6, 6.8, and 11.1 years, respectively. The average summary index in SNAC and non- SNAC groups was 0.803 and 0.819, respectively. Our results also showed a significant correlation between advanced osteoarthritis and proximal fracture non-unions(P<0.05). We concluded that there is no clear correlation between the interval and the progression of osteoarthritis. SNAC was more likely to occur in fractures aged 2 years or older

Osteomyelitis and septic arthritis are common pathologies in young children. Because of their skeletal immaturity, children are particularly vulnerable to orthopaedic complications, including limb-length discrepancies, angular deformities, chondrolysis, etc. The primary objective of this study was to review the clinical follow up and outcomes of paediatric patients diagnosed with osteoarticular infections. The secondary purpose was to look for significant differences in the clinical characteristics between the one with and without complications. Patients' medical charts, hospitalised between 2010 and 2016, were retrospectively reviewed. The inclusion criteria were: patients (1) aged of less than 10 years old (2) treated and followed for osteomyelitis of long bones of upper and lower extremities and/or septic arthritis (3) with at least one year of radiological follow up. The exclusion criterion was: (1) any concomitant chronic diseases. The information collected included demographic and clinical data. A late sequela was defined as a limb-length discrepancy superior to 5 mm or an abnormal articular angulation of more than 5°, or a symptomatic chondropathy. Patients were separated in two groups: with and without complications. Chi-square tests were used for categorical variables and Mann-Whitney U tests for continuous data in order to establish significant differences between both groups. Of the 401 patients with osteomyelitis and/or septic arthritis treated in our tertiary paediatric hospital over 7 years, 50 met the inclusion criteria. There were 24 girls and 26 boys. The etiological agent was identified in 56% of the cases. Staphylococcus aureus was the predominant causal pathogen (50%), followed by Kingella kingae (19.2%). The mean follow up was 780 days. Six out of 50 (12%) patients had physeal or chondrolytic complications at the latest follow-up. The only significant difference between the 2 groups was the delay between onset of symptoms and initiation of antibiotic therapy (P = 0.039). Only 12.5% of the patients were followed up at least one year. In the population of 50 skeletally immature patients without comorbidities, 12% had a sequela. The delay in initiating antibiotic treatment was significantly longer in the group with the presence of sequelae. The results of this study reveal that there were low rates of outpatient follow-up reaching more than a year after an osteoarticular infection, thus raising the question about the importance of a follow up after such a diagnosis. Twelve percent of the patients had a growth or chondrolysis complication and this might be related to the delay before initiating antibiotic treatment

Short scarf osteotomy (SSO) retains the versatility of standard scarf in treating moderate and severe hallux valgus deformity with the added benefit of less invasiveness translated into less soft-tissue stripping, reduced exposure, less metalwork, less operative time and reduced cost. We present our medium-term clinical, radiographic and patient satisfaction results. All patients who underwent SSO between January 2015 and December 2017 were eligible (98). Exclusion criteria were: follow up less than a year, additional 1st ray procedures, inflammatory arthropathy, infection, peripheral vascular disease and hallux rigidus. Eighty-four patients (94 feet) were included: 80 females / 4 males with average age of 51-year-old (24–81). Minimum follow up was 12 months (12–28). Weight-bearing x-rays and AOFAS score were compared pre- and postoperatively. Non-parametric Mann-Whitney U test assessed statistical significance of our results. Hallux valgus angle (HVA) improved from preoperative mean of 30.8° (17.4°–46.8°) to 12° (4°–30°) postoperatively (p=0.0001). Intermetatarsal angle (IMA) improved from preoperative mean of 15.1° (10.3°–21.1°) to 7.1° (4°–15.1°) postoperatively (p=0.0001). Average sesamoid coverage according to Reynold's tibial sesamoid position improved from average grade 2.18 (1–3) to 0.57 (0–2) (p=0.0001). Average AOFAS score improved from 51.26 (32–88) to 91.1 (72–100) (p=0.0001). Ninety percent of patients were satisfied and 83% wound recommend the surgery. No troughing phenomenon or fractures. Four overcorrections were found 3 of which did not require surgery. One recurrence at 18 months was treated with standard scarf. We believe that this technique offers a safer, quicker and equally versatile way of dealing with Hallux Valgus

The reduction for unstable femoral intertrochanteric fracture should be extramedullary, which means that the proximal fragment protrudes for the distal fragment. However, only few articles have compared extramedullary and intramedullary reductions in a biomechanical study. Thus, we created unstable femoral intertrochanteric fracture models using imitational bone (extramedullary and intramedullary groups, each with 12 cases) and evaluated their biomechanical stabilities. The fracture type was 31-A2 according to the AO-OTA Classification of Fractures and Dislocations and greatly lacked bone on the posterior side. We performed compression examination and evaluated stiffness. The implant used for fixation was TFNA (DePuy Synthes). We applied axial compression with 20 adduction in the standing position. Statistical analysis was performed using the Mann-Whitney U test. No significant difference in initial loading force was found between the two groups. However, the axial stiffness of the extramedullary bone showed a significant increase (p < 0.05) in high loading force (800–1000 N). This means that the stability of the extramedullary reduction was superior to that of the intramedullary reduction in terms of high loading force in the standing position. We suggest that antero-medial bony buttress is important for unstable femoral intertrochanteric fractures. These data indicate that extramedullary reduction and fixation for unstable femoral intertrochanteric fractures increase stability

It is well documented that implant loosening rate in sickle cell disease patients is higher than that seen in patients with hip arthroplasty from other indications. The Hypoxic inducible factor(HIF) - is activated in the microcellular hypoxic environment and this through a cascade of other enzymatic reactions promotes the activity of other factors and further help enhance angiogenesis and osteogenesis. The aim of this study was to investigate and propose a potential model for investigating osseointegration in a hypoxic microcellular environment using osteoblasts(MG63). Human MG63 osteoblastic cells were cultured under normoxia and hypoxic conditions (20%; and 1% oxygen saturation) for 72 hours under two different condition- with and without cobalt chloride. The samples cultured under normoxic condtions without cobalt chloride acted as control. Using qualitative polymerase chain reaction-(qPCR) - HIF expression was assessed under the above conditions in relation to the control. The results showed there was significant expression of the HIF 1 alpha protein under hypoxic condition with cobalt chloride in comparison with the control samples- all at 72hours incubation. Mann-Whitney U test was used to deduce level of significance of fold change.(p=0.002; <0.05). This was deemed as being a significant difference in the level of expression of HIF compared to the control. The results show that the hypoxic inducible factor can be expressed using the above tested. experimental invitro-model with significant results which can be a foundation for further research into improving hip implant prosthesis design to help enhance osseo-integration in sickle cell disease patient with AVN

Intraosseous Transcutaneous Amputation Prosthesis (ITAP) is a new generation of limb replacements that can provide to amputees, an alternative solution to the main problems caused by the most common used external prosthesis such as pressure sores, infections and unnatural gait. ITAP is designed as one pylon osteointegrated into the bone and protruding through the skin, allowing both the mechanical forces to be directly transferred to the skeleton and the external skin being free from frictions and infections. The skin attachment to the implant is fundamental for the success of the ITAP, as it prevents the implant to move and consequently fail. In this study we wanted to test if cell viability and attachment was improved using TiO2 nanotubes. Human keratinocytes and human dermal fibroblasts were seeded for three days on TiO2 nanotubes with different sizes (18–30nm, 40–60nm and 60–110nm), compared with controls (smooth titanium) and tested for viability and attachment. A Mann-Whitney U test was used to compare groups where p values < 0.05 were considered significant. The results showed that the viability and cell attachment for keratinocytes were significantly higher after three days on controls comparing with all nanotubes (p=0.02), while attachment was higher on bigger nanotubes and controls. Cell viability for fibroblasts was significantly higher on nanotubes between 40 and 110nm comparing with smaller size and controls (p=0.03), while investigation of cell attachment is ongoing. From these early results, we can say that TiO2 nanotubes can improve the soft tissue attachment on ITAP. Further in-vitro and ex-vivo experiments on cell attachment will be carried out

Background. The decision to resurface the patella during total knee arthroplasty remains controversial. Aim of our study was to evaluate the functional difference between patients undergoing medial rotation knee (MRK) replacement with and without patellar resurfacing at our hospital. Methods. We did a retrospective analysis of patients undergoing MRK total knee replacement (Matortho) at our hospital between 2008 and 2017 performed by 2 surgeons. Patients were recalled for a clinical review from Oct 2017 for recording of Oxford knee, Baldini and Ferrel scores. Mann-Whitney U test was used for non-parametric data (SPSS v24). Results. Of the 104(49 males) patients, 62 had patellar resurfacing. Age and sex distribution was similar in both groups. The mean follow up period was 74.45 months in non- resurfaced group and 54.93 months in resurfaced group. The Median (Range) pre-operative Oxford knee scores were similar in both groups − 15(4–42) in non-resurfaced group and 14(1–44) in resurfaced group. The median OKS at follow-up were 36(12–47) in non-resurfaced group and 37(9–48) in resurfaced group. The Patellar scores were slightly better in the resurfaced group – Baldini score median (range) (90 (25–100) in non resurfaced v/s 100(30–100) in resurfaced), Ferrel score (median (range) 25(12–30) in non-resurfaced v/s 28(10–30) in resurfaced, p 0.042). The patellofemoral component of the OKS (Q5 + Q7 + Q12) median (range) showed an improvement from 3(1–11) to 6.5 (3–11) in non-resurfaced group and from 3(0–12) to 8 (2–12) (p 0.039) in resurfaced group. Conclusion. Although the overall functional knee scores in non-resurfaced and resurfaced groups were same, we found a statistically significant difference in Ferrel score and in the patellofemoral component of OKS between the 2 groups of MRK knee replacement suggesting specific benefits of patellar resurfacing

Introduction. Acetabular dysplasia cause hip joint osteoarthritis(OA) by change hip mechanism. However, to our best knowledge, no studies have been published using prospectively collected data from asymptomatic young age volunteers, precise radiographic method. The purpose of this study is to evaluate the prevalence of hip dysplasia in asymptomatic Korean population as one of the most important risk factor of hip OA. Materials & Methods. From December 2014 to March 2015, we investigated prospectively collected retrospectively reviewed data of 200 asymptomatic volunteers 400 hips in age between 18 and 50 years recruited from our institution. Pelvic radiographs were taken and all radiographs were reviewed by 2 experienced orthopedic surgeons. Lateral center-edge angle(LCEA), Sharp angle, Tonnis angle and acetabular width-depth ratio were measured. We analyzed the statistical differences of these values between sex by Mann-Whitney U test and independent t-test. Pearson's correlation coefficient was used to measure the relationship between dysplasia parameters. Results. On the Pelvic AP view, 60 of the 400 hips (15%) were dysplastic hip as LCEA <20°. In 146 male hips, 17 hips (11.6%) were LCEA <20°. In 254 female hips, 43 hips(16.9%) were LCEA <20°. There was no strong correlation of LCEA with other measurements. Conclusion. There are large number of asymptomatic dysplastic hips in Asian population compared previously investigated

Objectives. Mesenchymal stem cells have the ability to differentiate into various cell types, and thus have emerged as promising alternatives to chondrocytes in cell-based cartilage repair methods. The aim of this experimental study was to investigate the effect of bone marrow derived mesenchymal stem cells combined with platelet rich fibrin on osteochondral defect repair and articular cartilage regeneration in a canine model. Methods. Osteochondral defects were created on the medial femoral condyles of 12 adult male mixed breed dogs. They were either treated with stem cells seeded on platelet rich fibrin or left empty. Macroscopic and histological evaluation of the repair tissue was conducted after four, 16 and 24 weeks using the International Cartilage Repair Society macroscopic and the O’Driscoll histological grading systems. Results were reported as mean and standard deviation (. sd. ) and compared at different time points between the two groups using the Mann-Whitney U test, with a value < 0.05 considered statistically significant. Results. Higher cumulative macroscopic and histological scores were observed in stem cell treated defects throughout the study period with significant differences noted at four and 24 weeks (9.25, . sd. 0.5 vs 7.25, . sd. 0.95, and 10, . sd. 0.81 vs 7.5, . sd. 0.57; p < 0.05) and 16 weeks (16.5, . sd. 4.04 vs 11, . sd. 1.15; p < 0.05), respectively. Superior gross and histological characteristics were also observed in stem cell treated defects. Conclusion. The use of autologous culture expanded bone marrow derived mesenchymal stem cells on platelet rich fibrin is a novel method for articular cartilage regeneration. It is postulated that platelet rich fibrin creates a suitable environment for proliferation and differentiation of stem cells by releasing endogenous growth factors resulting in creation of a hyaline-like reparative tissue. Introduction. The global rapid growth of the aging population has some likelihood to create a serious crisis on health-care and economy at an unprecedented pace. To extend Healthy Life Expectancy (HALE) in a number of countries, it is desired more than ever to investigate characteristic and prognosis of numerous diseases. This enlightenment and recent studies on patient-reported outcome measures (PROMs) will drive the increasing interest in the quality of life among the world. The demand for primary THAs by 2030 would rise up to 174% in USA. It is expected that the number of the elderly will surge significantly in the future, thus more septuagenarian and octogenarian are undergoing THA. Moreover, HALE of Japanese female near the age of 75 years, followed to Singapore, is still increasing. Therefore, concerns exist about the PROMs of performing THA in this age-group worldwide. Nevertheless almost the well-established procedure, little agreement has been reached to the elderly. We aimed to clarify the mid-term PROMs after THA over 75-year old. Methods. Between 2005 and 2013, we performed 720 consecutive primary cemented THAs through a direct lateral approach. Of these, 503 female patients (655 hips) underwent THA for treatment of osteoarthritis, with a minimum follow-up of 5 years, were retrospectively enrolled into the study. We excluded 191 patients (252 hips) aged less than 65-year at the time of surgery and 58 patients (60) because of post-traumatic arthritis or previous surgery (37), or lack of data (23). Thus, 343 hips remained eligible for our study, contributed by 254 patients. We investigated Quality-adjusted life year (QALY), EuroQol 5-Dimension 5-Level scale (EQ-5D) and the Japanese Orthopaedic Association Hip-Disease Evaluation Questionnaire (JHEQ, which was a disease-specific and self-administered questionnaire, reflecting the specificity of the Japanese cultural lifestyle) in patients aged 75 years or older (154 hips, Group-E) compared with those aged 65 to 74 years (189 hips, Group-C) retrospectively. We evaluated the association between patients aged 75 years or older and the following potential risk factors, using logistic regression analysis: age, number of vertebral fractures (VFs), American Society of Anesthesiologists physical status (ASA-PS) and Charlson Comorbidity Index (CCI). A p value of < 0.05 was considered significant for the Mann-Whitney U test. Results. At a mean follow-up duration of 7.2 years, QALY, EQ-5D and JHEQ for the domain of patient satisfaction were significantly greater for Group-E than Group-C; however, there were no significant differences in JHEQ for pain, movement and mental-health between groups. On multivariate analysis, the age (odds ratio [OR] 2.48, p < .01 for EQ-5D; OR .32, p < .01 for JHEQ satisfaction), VFs (OR 1.63, p < .01 for satisfaction) and ASA-PS (OR .64, p = .31 for EQ-5D) were independent predictive risk factors for patients aged 75-year or older. Conclusions. Based on mid-term follow-up of PROMs study, we suggest that cemented THA can lead to the extension of HALE towards the super aged society and our results can be applied to a systematic analysis for the Global Burden of Disease Study related frailty

Introduction. Interferon (IFN) based treatments for chronic hepatitis C (HCV) have been the standard of care until 2014 when direct antiviral agents (DAA) were introduced. Patients with HCV have had extremely high complication rates after total hip arthroplasty (THA). It is unknown whether HCV is a modifiable risk factor for these complications prior to THA. The purpose of this study was 1) to compare perioperative complication rates between untreated and treated HCV in THA and 2) to compare these rates between patients treated with two different therapies (IFN vs. DAA). Methods. A multicenter retrospective database query was used to identify patients diagnosed with chronic hepatitis C virus who underwent total hip arthroplasty from 2006–2016. All patients (n=105) identified were included and were divided into two groups: untreated HCV (n=63) and treated (n=42); the treated group were further subdivided into those receiving IFN based therapies (n=16) or DAA therapies (n=26). Comparisons between the treated and untreated groups were made with respect to demographic data, comorbidities, preoperative viral load, MELD score, and all surgical (≤1 yr) and medical (≤90d) complications; a sub-group analysis of the treated patients was also performed. Separate independent t-tests were conducted for dependent variables that were normally distributed, and Mann-Whitney U tests were conducted for variables which were not normally distributed. Categorical variables were compared through the chi-square test of independence. The level of statistical significance was set at p<0.05. Results. Of the 105 patients, there was a greater number of HIV infected patients in the untreated group and a higher number of smokers in the treated group. Surgical complication rates were higher in the untreated group (25.4% vs. 4.8%; p<0.05) with a PJI rate of 14.3% in the untreated group. There were no differences in medical complications. A sub-group analysis of the different treatments for HCV did not reveal any differences in post-surgical complications. Discussion and Conclusion. Untreated HCV is traditionally associated with an extremely high rate of postoperative complications following THA. Treatment for HCV prior to THA appears to be associated fewer postoperative complications, primarily PJI. While further investigation is warranted, strong consideration should be given to treating patients for HCV prior to elective THA

Aims. Accurate and precise acetabular reaming is a requirement for the press-fit stability of cementless acetabular hip replacement components. The accuracy of reaming depends on the reamer, the reaming technique and the bone quality. Conventional reamers wear with use resulting in inaccurate reaming diameters, whilst the theoretical beneficial effect of ‘whirlwind’ reaming over straight reaming has not previously been documented. Our aim was to compare the accuracy and precision of single use additively-manufactured reamers with new conventional reamers and to compare the effect of different acetabular reaming techniques. Materials and Methods. Forty composite bone models, half high-density and half low-density, were reamed with a new 61 mm conventional acetabular reamer using either straight or ‘whirlwind’ reaming techniques. This was repeated with a 61 mm single use additively-manufactured reamer. Reamed cavities were scanned using a 3D laser scanner with mean diameters of reamed cavities compared using the Mann-Whitney U test to determine any statistically significant differences between groups (p<0.05) [Fig. 1). Results. Reaming errors were significantly higher in low-density bone compared to high-density bone for both reamer types and reaming techniques tested (61.9 mm (SD 0.7) vs 61.4 mm (SD 0.4), respectively; p=0.0045). Whirlwind reaming was significantly more accurate and precise than straight reaming using both conventional (61.3 mm (SD 0.1) vs 62.3 mm (SD 0.4), respectively; p<0.0001) and single use reamers (61.1 mm (SD 0.3) vs 61.7 mm (SD 0.7), respectively; p=0.0058) [Fig. 2]. The novel single use reamer was significantly more accurate than the unused conventional reamer, using both the straight (61.7 mm (SD 0.7) vs 62.4 mm (SD 0.4), respectively; p=0.0011) and whirlwind techniques (61.2 mm (SD 0.3) vs 61.3 mm (SD 0.1), respectively; p=0.0002) [Fig. 3]. Conclusion. This is the first study to our knowledge that has assessed acetabular reaming technique in both low and high density saw bones. Improved reaming accuracy and precision was seen in both devices tested when using the ‘whirlwind’ technique in both high-density and low-density bone models when compared to a straight reaming technique. The single use device assessed reamed a cavity size closer to its stated size (61mm) compared to conventional reamers. Based on this study we suggest using a careful “whirlwind” technique when performing acetabular reaming, and for the surgeon to pay particular attention when performing joint replacement in patients with reduced bone quality as there is likely to be more variability in acetabular reaming accuracy in these patients

Introduction. Patient satisfaction is an important outcome measure after total knee arthroplasty (TKA) and is the ultimate goal of surgery. However, patient satisfaction reflects a complex construct of the patient's personal expectations and preferences in addition to subjective evaluation of outcome after TKA. Multiple studies have found a patient dissatisfaction rate of approximately 20% at 1 year after TKA. The purpose of this study is to determine whether there is an association between a single-item validated TKA satisfaction score and patient-reported outcome measures (PROMs) at 3 time points (1, 2, and 5 years after TKA) and to determine if dissatisfaction rate after TKA varies over time. Methods. A multi-center, prospective cohort of 12,952 patients (8,078 patients were assessed at 1-year, 702 patients at 2-year, and 4,172 patients at 5-year) undergoing primary TKA were enrolled by 230 surgeons in 28 states between 2012–2015. Surgeons practices varied in size, reimbursement models, and geographic setting ensuring that the cohort included diverse patient populations and delivery models. Surgeons agreed to invite all TKA patients to participate and sporadic audits of surgical logs validated that all patients were invited and > 90% of patients were included. Demographic and clinical data [age, gender, body mass index (BMI), and modified Charlson co-morbidity index (CCI)] were collected. Patient-reported outcome measures (PROMs) were collected pre-op and post-op at 1, 2, and 5 years using an internet-based platform including the KOOS (total score, and pain, ADL, QoL sub scores), KOOS Jr, SF-36 (PCS and MCS). We used the single-item satisfaction scale which was tested and validated by the Swedish Knee Arthroplasty Registry. The patients' responses were made on 5-point Likert scale (very satisfied, somewhat satisfied, neutral, somewhat dissatisfied, very dissatisfied). Patients were classified into 2 categories: satisfied group for patients who answered satisfied or very satisfied and dissatisfied group for patients answered neutral, somewhat dissatisfied, or very dissatisfied. Univariate analysis of the difference between the satisfied and dissatisfied patients' groups was performed using Mann-Whitney U test for continuous variables and chi-square test for categorical variables. Logistic regression model was performed to study the correlation between the satisfaction and PROMs with 95% confidence interval. Results. Mean age was 66.6 years, 62.7% were female, and mean BMI was 31.6. The CCI was 0 in 55.1%, 1 in 22%, 2–5 in 12.6% and ≥ 6 in 10.3%. Cumulative revision rate was 1.29% at 2 years. The 30-day adverse events incidence was 2.5% while the 90-day adverse events incidence was 4.7%. The dissatisfaction rate was 18.1% at 1-year, 11.5% at 2-year, and 8.5% at 5-year (P<0.001, Chi-square). Dissatisfaction significantly affects younger patients (<55 years) (P=0.04, Chi-square) and patients with high Charlson comorbidity index >1 (P=0.001, Chi-square). Low post-operative KOOS Pain, KOOS ADL, KOOS total score, KOOS JR, SF-36 PCS, and SP-36 MCS scores were significantly associated with dissatisfaction (P<0.001). At 5 years follow-up, in patients with KOOS scores greater than 70, 1.3% of patients were dissatisfied; with KOOS 50–70, 16.3% were dissatisfied and when KOOS < 50, 62.2% are dissatisfied. Logistic regression showed significant correlation of satisfaction with postoperative KOOS pain, KOOS ADL, KOOS QoL, KOOS total score, KOOS JR, and SF-36 PCS (P< 0.001) at 1-year, 2-year, and at 5-year. The MCS was correlated with patient satisfaction only at 1-year (P< 0.001). Conclusion. The patient dissatisfaction rate 5 years after TKA is 8.1% in FORCE-TJR patients which is significantly lower than 18.1% at 1-year. Less improvement of PROM scores after TKA are significantly associated with patient dissatisfaction. Postoperative PROM scores are associated with patient satisfaction at 1-year, 2-years, and 5-years. When the 5-year post- op KOOS total score is >70, 98.7% of patients are satisfied and only 1.3% are dissatisfied. Patient satisfaction is an important outcome measure after TKA that can be determined by asking a single question. However, in order to understand why a patient is dissatisfied, KOOS scores (KOOS pain, KOOS ADL, KOOS QoL) that assess specific postoperative outcomes can assist in determining the reason for patient dissatisfaction after TKA

Introduction. The usefulness of markers of non-specific low back pain (NSLBP), including MRI derived measurements of cross-sectional area (CSA) and functional CSA (FCSA, fat free muscle area) of the lumbar musculature, is in doubt. To our knowledge, such markers remain unexplored in Lumbar Disc Degeneration (LDD), which is significantly associated with NSLBP, Modic change and symptom recurrence. This exploratory 3.0-T MRI study addresses this shortfall by comparing asymmetry and composition in asymptomatic older adults with and without Modic change. Methods. A sample of 21 healthy, asymptomatic subjects participated (mean age 56.9 years). T2-weighted axial lumbar images were obtained (L3/L4 to L5/S1), with slices oriented through the centre of each disc. Scans were examined by a Consultant MRI specialist and divided into 2 groups dependent on Modic presence (M) or absence (NM). Bilateral measurements of the CSA and FCSA of the erector spinae, multifidus, psoas major and quadratus lumborum were made using Image-J software. Muscle composition was determined using the equation [(FCSA/CSA)*100] and asymmetry using the equation [(Largest FCSA-smallest FCSA)/largest FCSA*100]. Data were analysed using Mann-Whitney U tests (p value set at). Intrarater reliability was examined using Intraclass Correlations (ICCs). Results. ICCs ranged between 0.74 and 0.96 for all area measurements, indicating excellent reliability. There was no significant difference in TCSA and FCSA asymmetry (P=0.1–1.0) and muscle composition (P=0.1–1.0) between M and NM groups. Conclusion. Modic change in the absence of pain does not appear to influence cross-sectional asymmetry or composition of the lumbar musculature. CSA remains a controversial marker. No conflicts of interest. Instability after total hip arthroplasty is the most common indication for revision arthroplasty and can be difficult to treat. The purpose of this study is to evaluate the outcomes associated with the use of a constrained acetabular component as a treatment for instability after hip arthroplasty. We reviewed the clinical and radiographic outcomes of 149 arthroplasties, that had been performed with use of a single design of constrained acetabular component between 2007 and 2012 at a single institution. Patient demographics and case specific data were collected The Mann-Whitney U test was used to assess continuous variables. Categorical variables were examined using the Chi-square test and Fisher's exact test when appropriate. Survival probability was calculated using the Kaplan-Meier method. The mean age at time of index surgery was 70 years, 65% were female, and mean BMI was 26.3. The average number of previous surgeries was 3.6. The constrained liner was cemented into a well-fixed cup in 40 hips (20%). In eighty-two (55%) hips the constrained component was implanted for the treatment of recurrent instability, and in sixty-seven (45%) hips it was implanted because the hips demonstrate instability during revision surgery. At an average duration of follow-up of 4.2 (2–7) years the overall revision rate was 10.6 % The constrained acetabular device eliminated or prevented hip instability in all patients except five; 3.3% had a new dislocation and six (4.0%) had failure of the retentive ring. Three revisions (2%) were performed for deep infection, and 2 (1.3%) for acetabular component loosening. Radiographic analysis revealed a non-progressive radiolucent line around the cup in 19 hips (12.7%). When stratified by patient age, survivorship for patients less than 65 years of age versus those greater than 65 years was similar. This study correlates with results of other papers in the literature looking at outcome of constrained tripolar type sockets. The focal constraint socket with a metal ring type design has a much greater failure rate (9–29%). Constrained liners remain an excellent option for hip instability in early to mid- term follow up

Introduction. In the last couple of years, several synovial biomarkers have been introduced in the diagnostic algorithm for a prosthetic joint infection (PJI). Alpha defensin-1 proved to be one of the most promising, with a high sensitivity and specificity. However, a major disadvantage of this biomarker is the high costs. Calprotectin is a protein that is present in the cytoplasm of neutrophils, and is released upon neutrophil activation. Its value has been established for decades as a (fecal) marker for inflammatory bowel disease. Aim. To determine the efficacy of synovial calprotectin in the diagnosis of a prosthetic joint infection. Methods. We prospectively collected synovial fluid (from hip, knee and shoulder) from patients with a proven PJI (n=15) and from patients that underwent a revision surgery without signs of a PJI (n=19). Patients with an active rheumatoid arthritis and/or gout were excluded from the study. Synovial fluid was centrifuged and the supernatant was used to measure calprotectin, by using a rapid, point of care test. This test was validated for synovial fluid analysis of calprotectin using an ELISA. A Mann-Whitney U test was used to calculate the difference between both patient groups. Results. The median calprotectin level was 899 mg/L (range 28–2120) for PJI versus 22 mg/L (range 0–202) for controls (p < 0.0001). With a cut-off value of 50 mg/L, synovial calprotectin has a high sensitivity of 93%, and a specificity of 84%. The positive and negative predictive values are 82% and 94%, respectively. Conclusions. Synovial calprotectin is a potentially valuable biomarker in the diagnosis of a PJI. With a point of care test, a rapid quantative diagnosis can be made within the operating room (results are obtained within 20 minutes), and could help in the decision making process to re-implant a prosthesis in a one stage procedure. In comparison to the currently available test (to measure alpha defensin-1), the measurement of calprotectin test is much cheaper (500 euro versus 20 euro per sample) and easily to implement in hospitals where this test is already available. Its diagnostic efficacy for exclusively low-grade PJI should be further elaborated

Background. Orthopedic trauma patients can have significant pain management requirements. Patient satisfaction has been associated with pain control and narcotic use in previous studies. Due to the multifactorial nature of pain, various injury patterns, and differences in pain tolerances the relationship between patient factors and narcotic requirements are poorly understood. The purpose of this study is to compare patient demographics for trauma patients requiring high doses of narcotics for pain control versus those with more minimal requirements. Methods. Our study sample included 300 consecutive trauma activations who presented to our emergency department during the 2015 calendar year. Opioids given to the patients during their hospital stay were converted to oral morphine equivalents using ratios available from the current literature. Patients were placed into two groups including those who were in the top 10% for average daily inpatient oral morphine equivalents and the other group was composed of the remaining patients. In addition to morphine equivalents, patient age, gender, injury severity score, length of stay, number of readmissions and urine toxicology results were also recorded. Injury severity score (ISS), morphine equivalents, and patient age were evaluated with the Shapiro-Wilk test of normality. Comparisons were performed with the Mann-Whitney U test. Between group comparisons for positive urine toxicology screen and gender were performed with Chi square and Fisher exact test. Pearson correlations were calculated between injury severity score, average daily oral morphine equivalents, and length of stay. P-value of 0.05 was used to represent significance. Statistical comparisons were made using SPSS version 23 (IBM, Aramonk, NY). Results. Median average daily morphine equivalents in the 10% of patients receiving the highest doses was 86.30 and 12.95 for the bottom 90%. The difference was statistically significant (p<0.001). The median ISS between the 2 groups was significant (p=0.018). There was no significant difference in age, readmission rate, and urine toxicology results. Patients in the top 10% were more likely to be male (p=0.003). Median length of stay for the top 10% group and bottom 90% group was 4 days and 2 days, respectively (p=0.005). No correlation between injury severity score and length of stay was found (p=0.475). A weak correlation of 0.115 was found between morphine equivalents and length of stay (p=0.047). Discussion. Our study shows male gender and ISS were correlated with higher oral morphine equivalents for the 10% of patients receiving the highest daily amounts when compared to the reaminder of the cohort. There was a significantly increased length of stay in the patients receiving higher narcotic doses. Whether this is due to ISS or increase in narcotics is unclear. However, positive correlation was not found between ISS and length of stay

Introduction. The original Charnley-type negative pressure body exhaust suit reduced infection rates in randomized trials of total joint arthroplasty decades ago. Modern positive pressure surgical helmet systems (SHS) have not shown similar benefit, and several recent studies have shown a trend towards increased wound contamination and infection with SHS use. The gown glove interface may be one source of particle contamination. Objectives. The purpose of this study was to compare particle contamination at the gown glove interface in several modern SHS vs. a conventional gown. Methods. A 0.5 micron fluorescent powder was evenly applied to both hands to the level of the wrist flexion crease. After gowning in the normal fashion, the acting surgeon performed a standardized twenty minute simulated total joint replacement. The amount of gown contamination at the gown glove interface was then measured by three observers under ultraviolet light using a standardized grading scale; from 0 (no contamination) to 4 (gross contamination). Using Minitab 15, the Mann-Whitney U test was performed to compare gowns and an ordinal logistic regression analysis was performed to identify variables associated with levels of contamination. Results. All gown-glove interfaces showed some contamination. There was no difference among any of the gowns except for gown 2, which showed significantly more contamination when compared directly to each of the other four gowns (p<0.001 in each case) (Figure 1). The ordinal logistic regression analysis showed that gown type (p 0.10) was more significantly associated with contamination levels than were the other variables of observer (p 0.70), location of contamination (p 0.56), or trial order (p 0.5). Conclusions. Particle contamination occurs at the gown glove interface in most commonly used surgical helmet systems and was significantly increased in the gown with stiffer material that may be less apt to make a seal with the glove

D-dimer is one of the useful laboratory tests to evaluate the incidence of venous thromboembolism (VTE) after the total knee arthroplasty (TKA). The most recent guideline for the prophylaxis of VTE points out the surgical procedure itself is a major risk factor for developing VTE. Only a few literatures discuss the relationship of surgical procedures and the risk of venous thromboembolism. We therefore prospectively compare the difference of the perioperative plasma D-dimer levels between the patients undergoing navigation and convention TKA. Two hundred consecutive total knee arthroplasties were performed between September 2011 and March 2013. The patients were randomised according to their registration to the orthopaedic clinic. Ninety-six patients (100 knees) underwent a navigation-assisted TKA and ninety-four patients (100 knees) had a conventional TKA. No intramedullary violation was done in the navigation-assisted TKA, while the intramedullary femoral guiding was adapted in the conventional group. Pre-operative and post-operation day 1 plasma D-dimer levels were recorded and evaluated using Mann-Whitney U test. There was no difference in the demographic data and pre-operative D-dimer between the two groups (p=0.443). Significantly lower D-dimer levels on the post-operative day 1 were noted in the navigation group, when compared with the conventional group. (6.0 ± 4.4 mg/L vs 11.3 ± 9.6 mg/L, p = 0.000). We demonstrated that lower D-dimer level is developed after the navigation-assisted TKA than the conventional one. Less incidence of VTE is expected and the finding may help to explain the fact that less systemic emboli in the navigation assisted TKA

The Bundled Payments for Care Improvement (BPCI) initiative was introduced to reduce healthcare costs while maintaining quality. We examined data from a healthcare system comprised of five hospitals that elected to participate in the BPCI Major Joint Replacement of the Lower Extremity Model 2 initiative beginning July 1, 2015. We compared one hospital that did 507 BPCI knee cases to the four other hospitals that did 566 cases. Stratifying the data by hospital volume, we sought to determine if costs decreased during the BPCI period, how the savings were achieved, and if savings resulted in financial rewards for participation. Methods. The Medicare data included the target cost for each episode (based on historical data from 2009–2012 for each hospital that was adjusted quarterly) and actual Part A and Part B spending for 90 days. Using 1,836 primary knee replacements, we analyzed the costs associated with the anchor hospitalization, inpatient rehabilitation, skilled nursing facilities, home health, outpatient physical therapy and readmission to compare the 1,073 knees done during the 16-month BPCI initiative period with the 763 knees done during the 1-year period preceding BPCI participation. Owing to the nonparametric distribution of the cost data, a Mann-Whitney U test was used to compare the higher volume hospital with the four lower volume hospitals. Results. Compared to the preceding year, the mean episode of care cost during BPCI participation decreased by 8.5% (from $20,853 to $19,087, p=0.24) at the higher volume hospital while remaining virtually unchanged (going from $20,383 to $20,380, p=0.10) at the lower volume hospitals. During the BPCI period, the mean Medicare 90-day target cost was $18,307 at the higher volume hospital and $22,287 at the lower volume hospitals (p<0.001). At the higher volume hospital, the major components of the savings included $367,290 from reduced readmission rates (5.7% versus 8.7%, p=0.11), $207,608 primarily due to a reduction in the length of stay at skilled nursing facilities (mean 15 days versus 25 days, p=0.005), and $130,894 associated with a decreased percentage of patients using inpatient rehabilitation (3.2% versus 4.9%, p=0.22). Although offset by other cost increases, the largest component of the savings at the lower volume hospitals was $262,548 due to a decrease in the percentage of patients (2.3% versus 4.8%, p=0.04) using inpatient rehabilitation. Despite its savings, the mean reconciliation penalty was $851 per case at the higher volume hospital while the lower volume hospitals received a mean reward of $2,165 per case. Conclusion. Based on the reduction in costs and decreased readmission rates, the BPCI initiative is achieving its objectives. Despite an 8.5% decrease in costs, the $18,307 target based on historical data resulted in an $851 penalty per case at the higher volume hospital. In contrast, as a result of a $3,980 higher target, the lower volume hospitals were rewarded even though they did not achieve cost savings. As structured, there is no incentive for centers with historically low costs to participate in BPCI

There is evidence that fracture healing is delayed in severely injured patients. We recently demonstrated that a blunt chest trauma, which induced posttraumatic systemic inflammation, considerably impaired fracture healing in rats. Because the complement anaphylatoxin C5a is an important trigger of systemic inflammation, we tested the hypothesis, whether the impairment of fracture healing observed after a severe trauma resulted from systemically activated complement. 16 male Wistar rats received a thoracic trauma and a femur osteotomy stabilized by an external fixator. Immediately and 12 h after the trauma, half of the animals received a C5aR-antagonist to prevent the C5a-dependent systemic inflammation. Control rats received a nonsense peptide, which does not provoke any biological effect. The animals were killed after 35 days and the calli were analyzed by three point bending testing, μCT and histomorphometry. Statistics: Mann-Whitney U test, level of significance to p<0.05. The treatment with the C5aR-antagonist increased flexural rigidity significantly by 55%, improved bony bridging of the fracture gap and led to a slightly larger and qualitatively improved callus as evaluated by μCT and histological measurements. This study shows, that the immunomodulation by a C5aR-antagonist significantly reduced the deleterious effects of a thoracic trauma on fracture healing. C5a could possibly represent a target to prevent delayed bone healing in patients with severe trauma

Eligible patients were randomly allocated to PMI or standard intramedullary jigs. Smith and Nephew's patient specific cutting blocks (Visionaire) were used for PMI. Postoperative component positioning was investigated using the ‘Perth CT protocol’. Deviation of more than 3° from the recommended position was regarded as an outlier. Exact Mann-Whitney U test was used to compare component positioning and difference in proportion of outliers was calculated using Chi Squared analysis. Fifty-five knees were enrolled in the standard instrumentation group and fifty-two knees in the PMI group. Coronal femoral alignment was 0.7 ± 1.9° (standard) vs 0.5 ± 1.6° (PMI) (P=0.33). Outliers 9.4% vs 7.4% (P=0.71). Coronal tibial alignment was 0.4 ± 1.5° (standard) vs 0.6 ± 1.4° (PMI) (P=0.56). Outliers 1.9% vs 1.9% (P=0.99). Sagittal femoral alignment was 0.6 ± 1.5° (standard) vs 1.3 ± 1.9° (PMI) (P=0.07). Outliers 3.8% vs 13.2% (P=0.09). Tibial slope was 1.7 ± 1.9 ° (standard) vs 1.8 ± 2.7° (PMI) (P=0.88). Outliers 13.2% vs 24.1% (P=0.15). External rotation of femoral component was 0.6 ± 1.4° (standard) vs 0.2 ± 1.8° (PMI) (P=0.14). Outliers: 3.8% vs 5.6% (P=0.66). Compared to standard intramedullary jigs, patient matched instrumentation does not improve component positioning or reduce alignment outliers

We undertook a retrospective cohort study to determine if body mass index (BMI) is an independent factor in determining patient outcome following total hip replacement (THR). Outcomes were determined using the Oxford Hip Score, a questionnaire designed to evaluate self-reported function and pain levels in people with hip pain. We looked at all THRs carried out on patients with a BMI of ≥30 over a 38 month period (220 operations) and compared these with 220 randomly selected THRs carried out on patients with a BMI of <30 within the same 38 month period. For each case we recorded patient BMI, P score and Oxford scores. We calculated the difference between pre-operative and one-year post-operative Oxford hip scores. We used Kendall's rank correlation to assess any potential correlation between BMI and functional change, and the Mann-Whitney U test to establish if functional change between the groups was statistically significant. Our study indicates that BMI is not an independent factor in determining relative subjective functional change following THR. Although it showed that patients with a BMI of ≥30 have worse function both before and after THR, there was no statistically significant difference between the two groups in terms of functional change. We therefore suggest that patients with a higher BMI be considered candidates for THR as they can expect the same degree of functional improvement. Given the trend for increasing numbers of obese patients in the population these findings could have a significant impact on the future of THR operations

Because life expectancy is increasing, the number of primary knee arthroplasties performed is projected to increase 673% by 2030, according to Westrich et al. Also, Toulson et al. in a recent study predict that the incidence of deep infection associated with primary total knee arthroplasty ranges from 1% to 2%. Periprosthetic knee infection is one of the most dramatic and difficult to manage complications following total knee arthroplasty. Therefore, periprosthetic knee infection will continue to be a significant complication and an economic burden in the future. Our objective was to identify the risk factors that may provide greater likelihood of infection and thus select high-risk patients and to take maximum prevention strategies. Case-control study, between infected and non infected patients, undergoing primary total knee arthroplasty between January 2008 and January 2013. The risk factors evaluated were: duration of hospital stay, surgery duration, prophylactic antibiotics and timing for administration, volume of blood transfusion, autologous blood recovery system use, anesthetic technique, ASA classification, Diabetes Mellitus, Obesity (BMI>30), immunosuppression and history of any infection in the month preceding surgery. The presence of infection was defined by the criteria of the Center for Disease Control for Nosocomial Surgical Site Infections1. Statistical analysis IBM SPSS Statistics 20 (Fisher's exact test, Mann-Whitney U test and Student's t-test). Statistical significance for p ≤ 0.05. We evaluated 540 patients with a mean follow-up of 56 months. We identified 21 deep infections (3,8%), and 35 superficial wound infections and found a positive correlation between infection and obesity (p <0.01), immunosuppression (p <0.01), volume of blood transfusion (p=0.02), history of any infection in the month preceding surgery (p <0.01). We found a negative correlation with the use of a autologous blood recovery system (p <0.01). Other factors, commonly referred in the literature, showed no association or did not reach statistical significance. The incidence of periprosthetic knee infection after primary total knee arthroplasty stays high. The presence of obesity, immunosuppression, blood transfusion, history of any infection in the month preceding surgery were identified as significant risk factors for infection to occur. The identification, modification or eviction of the risk factors implied are essential to reduce and prevent infection in arthroplasty

Introduction. Patients with hip osteoarthritis have a substantial loss of muscular strength in the affected limb compared to the healthy limb preoperatively, but there is very little quantitative information available on preoperative muscle atrophy and degeneration and their influence on postoperative quality of life (QOL) and the risk of falls. The purpose of the present study were two folds; to assess muscle atrophy and degeneration of pelvis and thigh of patients with unilateral hip osteoarthritis using computed tomography (CT) and to evaluate their impacts on postoperative QOL and the risk of falls. Methods. We used preoperative CT data of 20 patients who underwent primary total hip arthroplasty. The following 17 muscles were segmented with our developed semi-automated segmentation method: iliacus, gluteus maximus, gluteus medius, gluteus minimus, rectus femoris, tensor facia lata, adductors, pectinus, piriformis, obturator externus, obturator internus, semimenbranosus, semitendinosus, vastus medialis and vastus lateralis/intermedius (Fig. 1). Volume and radiological density of each muscle were measured. The ratio of those of affected limb to healthy limb was calculated. At the latest follow-up, the WOMAC score was collected and a history of falls after surgery was asked. The average follow- up period was 6 years. Comparison of the volume and radiological density of each muscle between affected and healthy limbs was performed using the Wilcoxon signed rank test. Correlations between the volume and radiological density of each muscle and each score of the WOMAC were evaluated with Spearman's correlation coefficient. The volume and radiological density of each muscle between patients with and without a history of falls were compared using Mann-Whitney U test. Results. 13 of 17 muscles showed significant decrease in muscle volume in affected limb compared to healthy limb. The mean muscle atrophy ratio was 18.6±7.1 (SD) % (0–28.3%). Iliacus, psoas, adductors and piriformis showed a significant volume reduction more than 25 %. All 17 muscles showed reduced radiological density along the affected limb compared to the healthy side. The difference was 8.7±4.2 (SD) Hounsfield units (3.2 to 16.4). Gluteus medius and gluteus minimus showed a significant decrease of radiological density more than 15 HU. The radiological density of gluteus minimus showed higher correlation (R>0.7) with physical function scores of WOMAC for descending stairs, rising from sitting, walking on flat surface, going shopping and rising from bed. Seven of 20 patients had a history of falls, who showed significant reduced radiological density of gluteus minimus and obturator internus compared to the 13 patients without a history of falls. Conclusion. Almost all muscles of pelvis and thigh along the affected limb showed marked atrophy and fatty degeneration compared to the healthy side. Especially, the degree of fatty degeneration of gluteus minimus showed significant impacts on postoperative physical function and the risk of falls of patients

Introduction. Proper positioning of the baseplate and optimal screw placement are necessary to avoid loosening or failure of the glenoid component in RTSA. Several in vitro and anatomic studies have documented ideal number, size, length and angulation of baseplate screws. However, such fixation can often be tenuous, as the anatomy of scapula bone varies. Furthermore, it can be difficult to identify regions with the best bone stock intraoperatively even though surgeons have an understanding of scapular anatomy with potential screw trajectories in mind. This often leads to variable screw lengths and angulations in the clinical setting. The purpose of this study was to measure optimal screw lengths and angles to reach ideal regions in cadaveric scapulae and to compare the clinical experiences of three surgeons with each other and against a cadaveric model with screw lengths and angulations. Materials and Methods. Seven cadaveric scapulae were used as the template for optimal screw angulation and length for baseplate implantation. Total 21 cases (seven cases of each 3 surgeons) of reverse total shoulder arthroplasty using the Aequalis®-Reversed shoulder prosthesis (Tornier, France) were included. Measurement of screw angulation was done on the AP and axillary views to account for the superior-inferior and the antero-posterior angulations, respectively. The screw lengths used on each scapula was recorded prior to insertion in cadavers and retrieved from the operative records in clinical cases. Screws directed anteriorly and superiorly were recorded as positive values while posteriorly and inferiorly directed screws were designated negative values. The significant differences in degrees of screw angulation and screw lengths among the 3 surgeon groups were calculated using the ANOVA, with the p value at 0.05. The Mann-Whitney U test was performed to evaluate the cadaver group against the surgeon groups. Results. In cadaveric specimens, the averages of the screw lengths used were 29.4 mm (anterior screw), 15.4 mm (posterior screw), 36.0 mm (superior screw), and 46.70 mm (inferior screw). The anterior screw was directed 6.9° inferior and 7.5° degrees posterior in reference to the central peg. The posterior screw direction was inferior (−5.0°) and posterior (−1.7°); Superior screw was directed superiorly (30.1°) and anteriorly (22.2°), while the inferior screw was aimed inferiorly (−15.3°) and posteriorly (−8.3°). In clinical cases, the differences in screw length among the 3 surgeon groups were not statistically significant. There was no significant difference in screw angulation among the 3 surgeons except posterior screw. Comparing cadaveric specimens from the clinical cases, the anterior screws were shorter and directed more superiorly and anteriorly in the patients, and the superior and inferior screws were directed less superiorly and inferiorly in the patient. Conclusion. We concluded that more vertical screw placement of the superior and inferior screws is necessary to obtain the ideal baseplate fixation

Introduction. Tendon disease and rupture are common in patients with diabetes and these are exacerbated by poor healing. although nanoscale changes in diabetic tendon are linked to increased strength and stiffness. The resistance to mechanical damage of a tissue may be measured using fatigue testing but this has not been carried out in diabetic tendon, although the toughness of diabetic bone is known to be reduced. The aim of this study was to measure the static fatigue behaviour of tendons from a streptozotocin (STZ)-induced rat model of diabetes, hypothesising that diabetes causes tendon to show lower resistance to mechanical damage than healthy tendon. Materials and Methods. Diabetic (n=3, 12 weeks post-STZ) and age-matched control (n=3) adult male Sprague Dawley rats were culled, tails harvested and stored at −80ºC. Following defrosting, fascicles (5 per animal) were carefully dissected, mean diameter measured using an optical micrometer and mounted in a Bose Biodynamics test machine using custom grips in a PBS bath. Static fatigue testing at 30 MPa to failure enabled both elastic modulus (initial ramp) and steady state creep rate (gradient at creep curve inflexion) to be measured. Data are reported as median ± interquartile range and pw0.05 using a Mann-Whitney U test was taken as significant. Results. Confirming previous reports, tendon from diabetic rats showed significantly higher elastic modulus (201 ± 68 MPa) than healthy (151 ± 62 MPa). Strain at failure showed no differences between groups. Tendon from diabetic rats showed significantly slower steady state creep (71 ± 44 μstrain s. −1. ) than healthy (691 ± 1000 μstrain s. −1. ). Discussion. These preliminary data show an order of magnitude larger resistance to mechanical damage in diabetic tendons, possibly associated with the previously reported increased packing and decreased fibril diameters. Energy-storing flexor tendons, the most commonly affected in diabetics, and the positional tendons tested here show similar fatigue behaviour when tested at the same fraction of “stress-in-life”. Further investigation is required into the cell tissue repair response in diabetes in order to link reduced rates of mechanical damage with the clinically increased risk of disease and rupture in diabetic patients

Introduction. The number of revisions of total knee replacements (TKR) increases annually. Because of reduced bone stock, stable fixation of the implant is important. The femoral and tibial components are usually cemented whereas stems can be placed either cemented or press-fit (hybrid construct). To assess the stability of revision TKR with either cemented or hybrid places implants a randomized controlled trial (RCT) was executed, by using radiostereometric analysis (RSA). The short-term results of this RCT showed no differences between the two groups in stability and clinical outcomes. Although there were no clinical or radiological signs of loosening, both groups showed implants micromotion > 1 mm or degree. These findings might indicate the possibility of loosening later in time; therefore, the current study investigated the stability of cemented versus hybrid-placed revision TKR 6.5 years after surgery. Additionally, clinical results were evaluated. Methods. Of the 32 patients in the original RCT, 23 (12 cement, 11 press-fit) were available for mid-term follow-up measures. RSA images taken at baseline, 6 weeks, 3, 6, 12 and 24 months postoperatively were used from the previous study. New RSA images were taken at median 6.5 years (range 5.4–7.3) postoperatively. Stability of the femoral and tibial implants was assessed by using model-based RSA software (RSAcore, Leiden, The Netherlands) to determine micromotion. Clinical results were evaluated using the Knee Society Score (KSS), the Knee injury and Osteoarthritis Outcome Score (KOOS), active flexion, and VAS pain and satisfaction. Stability and clinical outcome were compared between the two groups using independent t-tests or Mann-Whitney U tests when applicable. Results. The median total translation at 6.5 years was 0.37 (0.13–1.96) mm and median total rotation 0.62 (0.11 – 2.81)° for the femoral component. For the tibia component the median total translation was 0.41 (0.10 – 1.04) mm and the median total rotation 0.61 (0.09 – 1.99)°. There were no differences in total translation and total rotation of the femoral and tibial component between the two groups. Additionally, none of the clinical scores differed between the groups. Interestingly, in the group with cemented stems five tibia implants showed > 1 mm or degree migration compared to zero in the hybrid group (p=0.02; Figure 1). Conclusion. There was no difference in stability and clinical outcome between fully cemented and hybrid-placed revision TKR 6.5 years postoperatively. Until now micromotion >1 mm or degree in the tibial components of the cemented group has not yet resulted in re- revisions. The patients will be followed to examine the consequences of these amounts of micromotion in this type of implant in the long-term. Figure 1. We conducted a prospective, randomised, controlled trial of 45 patients (47 elbows), with tennis elbow, who underwent either a formal open release or a percutaneous tenotomy. All patients had pre- and post-operative assessment using the Disability of Arm, Shoulder and Hand (DASH) scoring system. Both groups were followed up for a minimum of 12 months. Statistical analyses using the Mann-Whitney U test and repeated measured ANOVA showed significant improvements for patient satisfaction (p = 0.012), time to return to work (p = 0.0001), improvements in DASH score (p = 0.001) and improvement in sporting activities (p = 0.046) in the percutaneous group. Those patients undergoing a percutaneous release returned to work on average three weeks earlier and improved significantly more quickly than those undergoing an open procedure. The percutaneous procedure is a quicker and simpler procedure to undertake and produces significantly better results

The Bundled Payments for Care Improvement (BPCI) initiative was introduced to reduce healthcare costs while maintaining quality. We examined data from a healthcare system comprised of five hospitals that elected to participate in the BPCI Major Joint Replacement of the Lower Extremity Model 2 initiative beginning July 1, 2015. We compared one hospital that did 439 BPCI hip cases to the four other hospitals that did 459 cases. Stratifying the data by hospital volume, we sought to determine if costs decreased during the BPCI period, how the savings were achieved, and if savings resulted in financial rewards for participation. Methods. The Medicare data included the target cost for each episode (based on historical data from 2009–2012 for each hospital that was adjusted quarterly) and actual Part A and Part B spending for 90 days. Using 1,574 primary hip replacements, we analyzed the costs associated with the anchor hospitalization, inpatient rehabilitation, skilled nursing facilities, home health, outpatient physical therapy and readmission to compare the 898 hips done during the 16-month BPCI initiative period with the 676 hips done during the 1-year period preceding BPCI participation. Owing to the nonparametric distribution of the cost data, a Mann-Whitney U test was used to compare the higher volume hospital with the four lower volume hospitals. Results. Compared to the preceding year, the mean episode of care cost during BPCI participation decreased by 14.1% (from $21,823 to $18,755, p=0.02) at the higher volume hospital and by 13.1% (from $32,138 to $27,940, p=0.02) at the lower volume hospitals. During the BPCI period, the mean Medicare target cost was $18,490 at the higher volume hospital and $25,021 at the lower volume hospitals (p<0.001). At the higher volume hospital, the major components of the savings included $526,028 from reduced readmission rates (11.5% versus 3.0%, p<0.001), $393,757 primarily due to reduced length of stay at skilled nursing facilities (mean 33 days versus 26 days, p=0.07), and $205,459 associated with a decreased percentage of patients using inpatient rehabilitation (6.6% versus 3.2%, p=0.03). At the lower volume hospitals, the major components of the savings included $1,002,447 associated with a reduction in the length of stay at skilled nursing facilities (mean 37 days versus 27 days, p<0.001), $487,356 associated with a decrease in the percentage of patients using inpatient rehab (11.6% versus 6.5%, p=0.01), and $355,695 associated with reduced readmission costs. Despite the savings, the mean reconciliation penalty was $184 per case at the higher volume hospital and $2,309 per case at the lower volume hospitals. Stratified by the type of hip replacement, reconciliation for elective primaries resulted in a mean reward of $848 per case for the 409 hips performed at the higher volume hospital and $2,497 per case for the 266 hips done at the lower volume hospitals. For hip fracture cases, reconciliation resulted in a mean penalty of $14,248 per case among the 30 hips done at the higher volume hospital and $8,932 per case for the 193 hips done at the lower volume hospitals. Conclusions. Based on the reduction in costs and decreased readmission rates, the BPCI initiative is achieving its objectives. Although the higher and lower volume hospitals achieved savings during BPCI participation, the target costs were lower than the actual costs resulting in a reconciliation penalty that was driven by the cost of hip fractures. The reconciliation rewards associated with elective primary THAs compared to the substantial penalties associated with hip fractures support the premise that these groups should have different targets costs

Purpose. The purpose of this study was to investigate the rate of return to work after total knee arthroplasty (TKA). Materials and Methods. 105 knees of 80 cases were tested after TKA surgery. 18 knees of 14 cases were men and 87 knees of 66 cases were women. The average age at the time of surgery was 71.5 years, and the average postoperative observation period was 25.5 months. All patients were with osteoarthritis of the knee. The use models were Hi-tech Knee II (Nakashima Medical, Okayama, Japan) CR type 99 knees and PS type 6 knees. The rate of return to work after surgery and employment rate of before and after surgery were examined for all cases. In addition,[1]age at surgery [2]ROM [3]JKOM score(Akai 2006) [4]Knee Society score (KSS Insall 1989) were examined in each case, and found the factors influenced on employment rate after TKA. The occupation was classified using major group of the International Standard Classification of Occupations (ISCO-08) defined by International Labour Organization (ILO) on 2007. Mann-Whitney U test was used for the statistical work. Results. Preoperative and postoperative employment state was shown in the Fig.1. 29 knees of 23 cases were in employment before surgery. In these cases, 22 knees of 18 cases had return to work after surgery (Group I) and 7 knees of 5cases had retired after surgery(Group II). 76 knees of 57cases were unemployed before and after surgery(Group III). Return to work rate was 78.3 percent after TKA. In the Group I, all cases returned to the same occupation before surgery. The average age of the Group III at the time of surgery was significantly higher than Group I and II(Fig.2). Compared KSS, postoperative functional score of the group I was significantly higher than Group II and III(Fig.3). Discussion. The cases employed before TKA were younger than the unemployed cases. In the cases returned to work after TKA, stairs rise-and-fall capability and walking ability were higher than the other cases. Conclusion. (1)The rate of return to work was 78.3 percent after TKA.(2)Age at surgery influenced the preoperative employment.(3)Stairs rise-and-fall capability and walking ability after surgery involved the return to work

The purpose of this study was to evaluate whether epidural fibrosis formation around the spinal cord was affected by endogenous oestrogen deficient state after lumbar laminectomy in the rats. Thirty-six 12-month-old adult female Sprague-Dawley rats were used in this study. Bilaterally ooferectomy were done in 18 rats. Rats were divided into two groups: oophrectomised (oestrogen deficient) group and sham operated (oestrogen maintained) group. Three weeks after the ooferectomy each rat underwent complete bilaterally laminectomy at the L2 and L3 vertebral levels (two levels per rat). The rats were randomly divided into three equal groups (12 rats in each group). The rats were sacrificed at four, eight, and twelve weeks postoperatively and the lumbar spine excised en bloc, fixed and decalcified. Section stained with hematoxylin and eosin and Masson’s trichrome were used to evaluate epidural fibrosis, acute inflammatory cells, chronic inflammatory cells and vascular proliferation. Sections were analyzed by investigator blinded to the study and graded on a five-point grading system. Statistic were performed using Mann-Whitney U test when compare two variable and Kruskal-Wallis test when compare more than two variables. Compared with the oopherectomised group, the sham operated group showed decreased rate of epidural fibrosis and higher acute and chronic inflammatory cells response at four and eight weeks but this was no statistically significant (p> 0.05). The results of this study revealed that endogenous oestrogen may decrease epidural fibrosis formation after lumbar laminectomy in the rats

Introduction. Standard image-free navigation systems have proven to improve the accuracy of components placement and reduce the proportion of outliers for lower limb mechanical axis in total knee arthroplasty (TKA). However, their disadvantages include a longer duration of surgery and pin-tract related complications. More recently, pinless navigation has been developed to address these shortcomings. This study aims to investigate the effectiveness of a new pinless navigation system (Zimmer® iAssist™ Knee) in TKA. Methods. Power analysis was performed. To detect a significant difference in the proportion of outliers for lower limb mechanical axis at a power of 0.80, a sample size of 50 patients would be required. Between May 2013 and April 2014, 50 patients diagnosed with osteoarthritis of the knee were divided into two surgical technique arms: 1) iAssist™; 2) conventional TKA. One Adult Reconstruction surgeon performed all the surgeries. The patients were prospectively followed up and postoperative long leg radiographs were taken at one month post-surgery. Two reviewers blinded to the surgical method performed the measurements on radiographic films. Five radiographic measurements were measured: 1) Hip-Knee-Ankle Angle (HKA); 2) Coronal Femoral-Component Angle (CFA); 3) Coronal Tibia-Component Angle (CTA); 4) Sagittal Femoral-Component Angle (SFA); and 5) Sagittal Tibia-Component Angle (STA), The Mann-Whitney U test was used to compare the two groups for quantitative variables while the Fisher's exact test was used for categorical variables. Results. There was no difference between the two groups for age, BMI, gender and side of operated knee (all p>0.05). There was also no difference in the duration of surgery, postoperative drop in haemoglobin level and transfusion incidence. The median HKA, CFA, CTA, SFA and STA were comparable between both groups [Fig. 1]. Similarly, there was no difference in the proportion of outliers for lower limb alignment (HKA) and components placement (CFA, CTA, SFA and STA) [Fig. 2]. Conclusion. This study does not show any statistical difference in the use of iAssist™ to reduce the proportion of outliers for lower limb alignment and component placement in TKA patients. However this is possibly contributed by the fact that all the surgeries were performed by a single experienced Adult Reconstruction surgeon instead of orthopaedic surgeons in general, thereby reducing the incidence of outliers with conventional technique. In keeping with other studies on pinless navigational systems, this study also found no significant difference in postoperative drop of haemoglobin levels with the use of iAssist™, however this study also found no significant difference in operative time when using iAssist™

Introduction. Herniated disc tissue removed at surgery usually appears degenerated, and MRI often reveals degenerative changes in adjacent discs and vertebrae. This has fostered the belief that a disc must be degenerated before it can herniate, which has medicolegal significance. We hypothesise that degenerative changes in herniated disc tissues differ from those found in tissues that have degenerated in-situ, and are consistent with being consequences rather than causes of herniation. Methods. Surgically-removed discs were examined using histology, immunohistochemistry and confocal microscopy. 21 samples of herniated tissues were compared with age-matched tissues excised from 11 patients whose discs had reached a similar Pfirrman grade of degeneration but without herniating. Degenerative changes were assessed separately in three tissue types (where present): nucleus, inner annulus, and outer annulus. Mann-Whitney U tests were used to compare ‘herniated’ vs ‘in-situ’ tissues. Results. Herniated tissues showed significantly greater cellularity (annulus), greater proteoglycan loss (outer annulus), greater neovascularisation (annulus), greater innervation (annulus) as judged by PGP 9.5 staining, greater expression of matrix-degrading enzymes MMP1 and MMP3 (inner annulus), but less cell clustering (outer annulus). Some similar but non-significant differences were seen in nucleus tissues. Interpretation. Herniated tissues that escape the pressurised confines of the disc are free to swell, lose proteoglycans and come into contact with blood cells. These events could explain most of the differences between herniated tissues and those that degenerated ‘in situ’. Results support our hypothesis, and warn against assuming that degenerative changes always precede (or cause) disc herniation. No conflicts of interest. No funding obtained. Purpose of study. Transarticular screw fixation offers acceptably high fusion rates but is not possible in 18% of patients due to a high riding vertebral artery. It also requires pre-operative anatomical reduction which is not always possible. The Harms' technique utilises a posterior C1 lateral mass and C2 pedicle screw. This allows easier access due to the angle of drilling and has become an increasingly popular surgical technique. The aim of this study is to review and compare the above techniques with regard to surgery, complications and outcome. Description of methods. This study is a retrospective chart and radiographic review of patients undergoing posterior C1-2 fusion in a single institution in the period 2003 to 2011. The most common aetiology was rheumatoid arthritis and post-traumatic instability. All atlanto-axial instability patients that came to surgery are included in this study, and only cases with less than six months follow-up were excluded. We report on surgical indications, surgical outcomes, complications and radiographic outcomes. Summary of results. No statistically significant differences were found in blood loss (p=0.47) or surgical time (p=0.44) using the Mann-Whitney U test. Complications in patients undergoing transarticular screw fixation included the need to abandon transarticular screws in two cases due to technical difficulty, metalware failure in two cases and intra-operative cerebrospinal fluid leaks in 2 cases. The Harms technique was associated with a single case of cerebrospinal fluid leak. Conclusion. There is no significant difference in surgical time and blood loss between the two techniques. Both are reliable in terms of fusion. The Harms technique offers the advantage of intra-operative reduction and a smaller wound due to the direction of access. The decision to use one or the other is based on the surgeons skill levels, ability to pre-operative reduce the joint and possibly the cost. NO DISCLOSURES

Introduction. Range of motion (ROM) simulation of the hip is useful to understand the maximum impingement free ROM in total hip arthroplasty (THA). In spite of a complex multi-directional movement of the hip in daily life, most of the previous reports have evaluated the ROM only in specific directions such as flexion-extension, abduction-adduction, and internal - external rotation at 0° or 90° of hip flexion. Therefore, we developed ROM simulation software (THA analyzer) to measure impingement free ROM in any positions of the hip. Recent designs of the hip implants give a wider ROM by increasing the head diameter and then, bone to bone impingement can be a ROM limit factor particularly in a combination of deep flexion, adduction and internal rotation of the hip. Therefore, the purpose of this study were to observe an individual variation in the pattern of the bone impingement ROM in normal hip bone models using this software, to classify the bone impingement ROM mapping types and to clarify the factors affecting the bone impingement type. Methods. The subjects were 15 normal hips of 15 patients. Three dimensional surface models of the pelvis and femur were reconstructed from Computer tomography (CT) images. We performed virtual hip implantation with the same center of rotation, femoral offset, and leg length as the original hips. Subsequently, we created the ROM mapping until bone impingement using THA analyzer. We measured the following factors influenced on the bone impingement map patterns; the neck shaft angle, the femoral offset, femoral anteversion, pelvic tilt, acetabular anteversion, sharp angle, and CE angle. These factors were compared between the two groups. Statistical analysis was performed with Mann-Whitney U test, and statistical significance was set at P<0.05. Results. According to the borderline of ROM at the flexion-internal rotation corner on the bone impingement map, the hips were classified into two groups; group-A showed more than 45° of the borderline slope at the flexion-internal rotation corner and the remaining hips were group-B. (Fig.1). There were 7 hips in group-A and 8 hips in group-B. Femoral offset was 36.8±2.2 mm in group-A and 30±2.7 mm in group-B. Femoral anteversion was 32±6.4° in-group A and 43 ±4.8° in group-B. There were statistically significant differences in the femoral offset and femoral anteversion between the groups. There were no significant differences in the other factors. Discussion. The results of this study showed various ROM map patterns even in normal hips and we classified them into two groups. An increased femoral offset or a decreased femoral anteversion revealed an early impinge in internal rotation. ROM until bone impingement is affected by the individual bone morphology. However, it is not easy to evaluate bony ROM in complex hip positions. THA analyzer shows the impingement position visually on the map and it is easy to understand the hip positions with reduced ROMs. Conclusion. There are two patterns on the bony ROM map in normal hips, and an early impinge in internal rotation occurred by increasing the femoral offset or decreasing the femoral anteversion. Background. Benefits of day case foot and ankle surgery includes reduced hospital stay, associated cost savings for the hospital, high patient satisfaction and quicker recovery with no increase in complication rates. In 2007, we set up the preoperative foot and ankle group. Patients were seen three weeks before surgery by a specialist nurse, physiotherapist and a preoperative evaluation is done. The therapist explains the patient's weightbearing status and advices on how to carry this out. Our aim was to reduce inpatient hospital stay and increase our day case procedures. Methods. We evaluated length of stay and physiotherapy intervention for all our patients during the first three months of 2007 to 2011. Mean length of stay was calculated and Mann-Whitney U test was performed using median. Results. Mean length of stay for combined forefoot and midfoot group reduced by 1.92 days and median reduction was statistically significant(p< 0.01). For forefoot surgery alone, the mean length reduced by 2.14 and median reduction was significant(p< 0.001) and for midfoot surgery alone, the mean stay reduced by 1.34 days and median was significant (p< 0.001). Hind foot patient's mean length of stay reduced by 6.78 days and the median was significant (p< 0.001). But for the ankle group the mean length of stay did reduce but the median was not statistically significant (p=0.225). Day case surgery increased by 43.5% for forefoot, 23.2% for midfoot and 14% for ankle surgeries but not for hindfoot. Conclusions. The overall results show that the preoperative foot and ankle group has resulted in reduction of inpatient stay and increase in daycase surgery performed. A pre-operative group is a highly efficient method of enhancing patient care and improving length of stay at the hospital for the patient. The cost saving for the hospital is around £35,400 per annum

Introduction. Demineralised Bone Matrix (DBM) is widely used in Orthopaedics and dentistry as a bone graft substitute and may be used to augment bone formation in load bearing applications. In this study we examine the effect of gamma irradiation and freeze drying on the tensile strength of Demineralised Cortical Bone (DCB). Methods. Tibias were harvested from mature ewes and cut into bony strips. Demineralisation was done using 0.6M HCL and confirmed by X-ray. Specimens were washed until a pH of 7.0 +/_ 0.2 was achieved in the washing solutions. Specimens were allocated into 4 groups; group (A) non freeze dried non gamma irradiated, group (B) freeze dried non gamma irradiated, group (C) non freeze dried gamma irradiated mention the level of gamma irradiation and group (D) freeze dried and gamma irradiated. The maximum tensile force and stress were measured. Statistical analysis using the Mann-Whitney U test was carried out. Results. The Median of maximum tensile force for group (A) was 218N, group (B) was 306N, group (C) was 263N and for group (D) was 676N. Group (D) results were statistically higher (p=< 0.05) compared to group (A) and (C), while there was no statistical significance compared to group (B). Conclusion. Previously published studies suggested the possibility of using DCB as ACL graft substitute. We examined the effect of gamma radiation as the most common sterilisation technique in medical field and the freeze drying as a possible technique for long term storage on the tensile strength of the DCB. Freeze drying significantly increases the tensile strength of the DCB while gamma irradiation has no significant effect. Our results indicate that freeze dried gamma irradiated DCB can be used as a ligament substitute

Introduction. Successful cementless acetabular designs require sufficient initial stability between implant and bone (with interfacial motions <150 μm) and close opposition between the porous coating and the reamed bony surface of the acetabulum to obtaining bone ingrowth and secondary stability. While prior generations of cementless components showed good clinical results for long term fixation, modern designs continue to trend toward increased porosity and improved frictional characteristics to further enhance cup stability. Objectives. We intend to experimentally assess the differences in initial stability between a hemispherical acetabular component with a highly porous trabecular tantalum fixation surface (Continuum. ®. Acetabular System, Zimmer Inc, Warsaw, IN)(Fig 1) and a hemispherical component with the new highly porous Trabecular Titanium. ®. surface (Delta TT, Lima Corporate, Italy)(Fig 2) manufactured by electron beam melting. Material and methods. A total of 16 cups were used, 8 for each type. Each cup was used 4 times. Cups were implanted in polyurethane foam blocks with 1mm interference fit and subsequently edge loaded to failure. Two different foam block densities (0.24 g/cm. 3. and 0.32g/cm. 3. ) were used to model low- and high-density bone stock. Each cup was seated into a block under displacement control using a servohydraulic test machine (MTS Bionix 858, Eden Praire, MN) to engage the locking mechanism until axial forces reach 8 to 10 kN. During insertion, force and displacement were recorded to determine the implantation force for each component. After seating, initial acetabular component fixation was assessed using an edge loading test. Descriptive statistics are presented as means and standard deviations for continuous variables. The Kruskal-Wallis test was used to assess the effect of Cup on the outcomes: (1) Insertion force, (2) Insertion energy, (3) Ultimate load, (4) Yield load, and (5) Ultimate Energy. Pairwise comparisons were done using Mann-Whitney U test for significant outcomes and multiple comparisons were adjusted using Bonferroni correction. All analyses were performed with SAS version 9.3 (SAS Institute, Inc., Cary, NC, USA); a p-value less than 0.05 was considered statistically significant. Results. Delta TT cup required the same seating force (p=0.014) and 18% higher insertion energy (p=0.002) for fully seating compared to Continuum cup, however this difference is not clinically relevant. Delta TT cup exibithed more stability, as exibithed by significantly higher (35%) energy to ultimate load (p=0.014). No statistical differences were found in Ultimate load and Yield load among the 2 cups. Cups in higher density foam required higher force and energy to be seated. In edge load testing higher densities blocks generated higher force and energy accross all cup designs. Conclusions. The result of this study indicate increased interface stability in Trabecular Titanium cup compared to Porous tantalum cup with a low incresing in the energy required for fully seating

Introduction. Because of the low cost and easy access, surgical video has become a popular method of acquiring surgical skills outside operating rooms without disrupting normal surgical flow. However, currently existing video systems all use a single point of view (POV). Some complex orthopedic procedures, such as joint replacement, require a level of accuracy in several dimensions. So single and fixed POV video may not be enough to provide all the necessary information for educational and training purposes. The aim of our project was to develop a novel multiple POV video system and evaluate its efficacy as an aid for learning joint replacement procedure compared with traditional method. Materials and Methods. Based on the videos of a hip resurfacing procedure performed by an expert orthopedic surgeon captured by 8 cameras fixed all around the operating table, we developed a novel multiple POV video system which enables users to autonomously switch between optimal viewpoints (Figure 1). 30 student doctors (undergraduate years 3–5 and naive to hip resurfacing procedure) were recruited and randomly allocated to 2 groups: experiment group and control group, and were assigned to learn the procedure using multiple or single POV video systems respectively. Before learning they were first asked to complete a multiple choicetest designed using a modified Delphi technique with the advice and feedback sought from 4 experienced orthopedic surgeons to test the participants' baseline knowledge of hip resurfacing procedure. After video learning, they were asked to answer the test again to verify their gained information and comprehension of the procedure, followed by a 5-point Likert-scale questionnaire to demonstrate their self-perception of confidence and satisfaction with the learning experience. The scores in the 2 tests and in the Likert-scale questionnaire were compared between 2 groups using Independent-Samples t-test (for normally distributed data) or Mann-Whitney U test (for non-normally distributed data). Statistical significance was set as p<0.05. Results. There was no significant difference regarding the ages of the participants between the experiment group (22.27 ± 1.79 years) and the control group (23.00 ± 1.56 years) (p value=0.242). The 10 questions in the test were divided into 3 subcategories: 4 questions regarding spatial awareness, 4 regarding operation details and 2 regarding sequence comprehension. There was no significant difference between both cohorts in the baseline test scores (for overall scores or scores in any subcategory) before video-learning, thus ensuring homogeneity. Yet, there was a 31.6–75.4% significantly (p<0.033) higher test score after video learning in the experiment group compared with the control group (for overall, spatial awareness and operation details scores) (Figure 2). The mean Likert-scale questionnaire score in the experiment group was also 32% significantly greater than the control group (Figure 3). Conclusion. Trainees could gain better knowledge and comprehension of hip resurfacing procedure and show higher confidence and satisfaction after learning using the novel multiple POV video system compared with traditional single POV video. The novel system could serve as an effective tool for teaching hip resurfacing procedure before trainees proceed to the real operating room. For figures, please contact authors directly

Informed consent is vital to good surgical practice. Pain, sedative medication and psychological distress resulting from trauma are likely to adversely affect a patient's ability to understand and retain information thus impairing the quality of the consent process. This study aims to assess whether provision of written information improves trauma patient's recall of the risks associated with their surgery. 121 consecutive trauma patients were randomised to receive structured verbal information or structured verbal information with the addition of supplementary written information at the time of obtaining consent for their surgery. Patients were followed up post-operatively (mean 3.2 days) with a questionnaire to assess recall of risks discussed during the consent interview and satisfaction with the consent process. Recall of risks discussed in the consent interview was found to be significantly improved in the group receiving written and verbal information compared to verbal information alone (mean questionnaire score 41% vs. 64%), p=0.0014 using the Mann-Whitney U test. Patient satisfaction with the consent process was significantly improved in the group receiving written and verbal information, 97.9% of patients reported that they understood the risks of surgery when they signed the consent form compared to 83.2% who received verbal information alone (p=0.01). The majority of patients who received written information reported finding it helpful (93.8%) and most of the patients who did not receive written information reported they would have found it useful had it been offered (66%). Patients awaiting surgery following trauma can pose a challenge to adequately inform about benefits conferred, the likely post operative course and potential risks. Written information is a simple and cost-effective means to improve the consent process and was popular with patients

Introduction. The incidence of dislocation after total hip arthroplasty (THA) was reported to be 0.5 to 10% in primary THA and 10 to 25 % in revision THA. The main causes of instability after THA were reported to be implant malalignment and inappropriate soft tissue tension. However, there was no study about quantitative data of soft tissue tension of unstable THA. The purpose of this study is to clarify the features of soft tissue tension of unstable THA in comparison to stable THA. Methods. The subjects were 15 patients with 15 THAs who had developed recurrent dislocation after primary THA. Thirty four patients with 37 THAs who developed no dislocation for one year after surgery were recruited as a stable THA group. In both group, all THAs were performed through posterolateral approach. In order to assess the soft tissue tension of THA, we recorded antero-posterior radiographs of the hips while applying distal traction to the leg with traction forces of 20?, 30%, 40% of body weight (BW). The distance of separation of the head and the cup after traction was measured under correction of magnification. Nine of 15 THAs in the unstable THA group and 32 of 37 THAs in the stable THA group were unilateral involvement. In the hips with unilateral involvement, the femoral offset difference between the healthy hip and the reconstructed hip were evaluated. Statistical analysis was performed with χ2 testand Mann-Whitney U test, and statistical significance was set at P<0.05. Results and Discussion. The average separation distance of the head and the cup was 5.2 ± 3.4mm (SD) at 40%BW, 4.3±3.2mm at 30%BW, and 3.2±2.8mm at 20%BW in the unstable THA group. The average separation distance of the head and the cup was 1.4±1.5mm at 40%BW, 1.1±1.4mm at 30%BW, and 0.9±1.2mm at 20%BW in the stable THA group. There were statistically significant differences in the separation distance between the groups in all ranges of traction force. The femoral offset difference between the operated side and the healthy side was −1.2±5.6mm in the unstable THA group and 3.1±4.8mm in the stable THA group. There were no significant difference in the femoral offset difference, however the femoral offset tends to be small in the unstable THA group compared to the stable THA group (P=0.05). The leg length discrepancy was −3.1±11.6mm in the unstable THA group and 2.7±7.1mm in the stable THA group. There were no significant difference in the leg length discrepancy (P=0.12). Conclusion. The separation distance of the head and the cup during leg distal traction in the unstable THA group is about four times larger than that in the stable THA group. The femoral offset tended to be smaller in the unstable THA group compared to the stable THA group

Aim. The recent discovery of small non-coding RNAs, so-called micro RNAs (miRNAs), has provided new insights into cancer diagnosis. Several studies have shown that profiles of miRNA expression differ between normal tissue and tumour tissue and vary among tumour types. To exploit this difference, we evaluated the feasibility of using muscle-specific miRNAs (miR-1, 133a, 133b, 206) as biomarkers of rhabdomyosarcoma (RMS). Method. Total RNA was extracted from 16 cell lines (7 RMS, 4 neuroblastoma, 3 Ewing sarcoma and 2 malignant rhabdoid tumour) and 21 tumour specimens (7 RMS, 1 Ewing sarcoma, 4 undifferentiated sarcoma, 1 osteosarcoma, 1 alveolar soft part sarcoma, 2 neuroblastoma, 2 Wilms tumour, 1 malignant rhabdoid tumour, 1 adrenal carcinoma and 1 retinoblastoma). miRNA was quantified by real-time RT-PCR. The expression levels of miRNAs were calculated utilizing the delta-delta Ct method, normalised to the level of miR-16, and compared using the Mann-Whitney U test. Results. The expression levels of muscle-specific miRNAs in the RMS cell lines were significantly higher (p<0.01) than those in neuroblastoma, Ewing sarcoma and malignant rhabdoid tumour cell lines. miR-206 was most abundantly expressed and miR-1 was least abundantly expressed among muscle-specific miRNAs in RMS cell lines. The expression levels of musclespecific miRNAs in RMS tumour specimens were significantly higher (p<0.01) than those in other pediatric tumours. The difference in the expression levels between RMS and other tumours was largest in miR-206. Conclusion. The expression levels of muscle-specific miRNAs were significantly elevated in RMS cell lines and tumour specimens. Muscle-specific miRNAs, especially miR-206, can be potential biomarkers for RMS diagnosis

Aim. The aim of this study was to determine the factors which were responsible for differences between patients achieving the Trust target of discharge on post-operative day 5 after a primary total knee replacement and those not achieving it, in the cohort of over 75 year olds. Methods and Results. Of all the patients undergoing a TKR at Addenbrooke's Hospital in 2008, those over 75 were identified (n=103). From the literature pre-, intra- and post-operative factors that had previously been shown to affect length of stay were identified. Patient notes were examined for details on each of these and the patients divided into 2 groups according to whether their discharge was achieved by day 5 or not. Data from 74 operations was available at the time of submission of this abstract. Pearson's Chi-squared test, student's independent t-test or the Mann-Whitney U test were performed on the data depending on the nature of the variable analysed. The following factors were found to be significantly different between the 2 groups at the 95% confidence level: pre-operative use of a walking aid (p=0.033), pre-operative Hb (p=0.003), post-operative Hb (p=0.001), post-operative requirement of a blood transfusion, post-operative complication (p<0.001), post-operative day on which active knee flexion to 90° was achieved (p=0.003). In addition the following factors were found to be significant at the 90% confidence level: age (p=0.082), comorbidity (p=0.086), marital status (p=0.095) and mobilisation by post-operative day 2 (p=0.082). Conclusion. Pre-operative use of any walking aid, peri-operative haemoglobin concentration and post-operative complications (including the need for a blood transfusion) seem to be the significant factors associated with a prolonged stay in hospital in the over 75 year olds. A few other factors are bordering on significance and they warrant further investigation in a larger patient cohort

Background: Measurement of quality of life (QOL) and functional status provides important additional information for priority setting in health policy formulation and resource allocation. Our aim was to define the differences in the health-related quality of life between hip artroplasties with cementation and without cementation. The last objective was to reunite evidences on the advantages and disadvantages of both systems of hip arthroplasty fixation. Methods: We analyzed a random sample of patients in surgical waiting list of total hip arthroplasty, between 65 and 75 years, divided in two groups of 40 patients who received a cemented or uncemented THA, respectively. We compared the pre-operative characteristics and at a year after operation changes in the Nottingham Health Profile (NHP) and SF-12 self-administered questionnaires. We also performed the specific Harris hip score. To make the different scoring systems comparable, all scores were transformed to a 0-to 100-point scale, with 100 points indicating best health. Differences among these groups were compared using the Mann-Whitney U test. Results: All patients increased their QOL scores. Both groups had similar QOL scores before surgery. At 1 year, patients with the uncemented prosthesis had slightly higher scores for energy, pain, and emotional reaction. Changes in QOL scores were, however, very similar. Conclusions: The use of an uncemented prosthesis does not impair early outcome

To identify the incidence and timing of superior subluxation following total shoulder replacement (TSR) and any associated change in pain, activities of daily living and ranges of movement. Forty-six TSR in rheumatoid patients with more than 5years follow-up were identified from a prospectively compiled database held by the senior author (ANS). Modified Constant scores (excluding the power component) were measured and recorded prospectively every 2years. Pre-operative and complete follow-up scores were available for 35 joints (27 patients). A Mann-Whitney U test was used to compare patients with subluxation and those without, with regard to the changes in the components of the Constant score at last follow-up compared with the pre-operative score. Superior subluxation of the humeral head was defined as when the lower third of the humeral head had migrated level or superior to the midpoint of the glenoid component as measured on the AP radiograph. An independent observer reviewed AP radiographs, taken at each 2 yearly review, at random with identity hidden and in no particular date order. Twenty-three patients developed superior subluxation since surgery, of which 87% occurred after 5years. Of the 35 joints with both clinical and radiological follow-up, 16 had evidence of subluxation. There was no statistically significant difference between the changes in the activities of daily living (Mann-Whitney U=106, p=0.1) and range of movement (U=140, p=0.7) components of the Constant score. However, patients without subluxation had a greater improvement in their pain scores (U=80, p=0.02). Approximately half of rheumatoid patients with TSR will demonstrate radiological changes of superior subluxation, in the majority after 5 years. This change is not associated with deterioration in activities of daily living or ranges of movement. However, pain relief persists irrespective of subluxation but is greater in those without subluxation


Aim of this study was to compare the postoperative range of motion of three types of total knee replacements. They were 72 posterior cruciate ligament retaining knee prostheses (group I), 61 postero-stabilized (group II), 52 ultracongruent plates (group III). Inclusion criteria were primary arthritis with varus deformity inferior to 15 degrees (°), no previous surgery on the knee, body mass index inferior to 35, preoperative flexion superior to 110°. All prostheses were performed with the same ancillary with one unique surgeon (DH). Recovery and analgesia protocols were similar in the three groups. Mobility was measured using a goniometer. Continuous data were tested for normal distribution using Kolmogorov-Smirnov test. Normally distributed data were analyzed with two tailed t-tests, whereas non-parametric data were analyzed with Mann-Whitney U test. Statistical significance was set at p < 0.05. At 2 years follow-up, the group I demonstrated a mean flexion of 121.8° in preoperative period and 110.3° in postoperative period. They were respectively of 121.3 and 122.2° in the group II. Regarding group III, they were 121.6 °preoperatively and 118.4° postoperatively. Results were significant (p< 0.05) between groups I and II, and groups I and III. No statistic correlation was found between need of mobilisation under general anesthesia (p> 0.05), flexum (p> 0.05), knee score (p> 0.05), patient satisfaction depending on type of prostheses (p> 0.05). Posterior cruciate ligament removal tends to offer a best postoperative flexion without significant influence on the knee score or patient satisfaction

[Purpose]. There have been only a few reports about the efficacy of postoperative cryotherapy following total hip arthroplasty (THA), and past studies have described that local cooling is efficacy for pain relief. The purpose of this study is whether the continuous local cooling following THA is effective for pain relief and the reduction of blood loss, swelling, and the duration of hospital stay. [Materials and Methods]. Thirty-eight patients (39 hips) underwent primary cementless THA for osteoarthritis and were divided into a cryotherapy group (30 subjects; from Apr. 2013 to Oct. 2013) and a control group (9 subjects; from Nov. 2012 to Mar. 2013). In the cryotherapy group, a continuous cooling pad was applied on the surgical wound and the thigh with a cloth anchor band (CF-3000, Sigmax, Japan) with the cooling temperature set to a constant 5°C for 72 hours immediately after surgery. Blood was collected on postoperative days 1,4,7,14, and 21 to determine Hb, CK, and CRP levels. Postoperative pain of the hip was scored by using a visual analog scale questionnaire on postoperative days 1 to 28. Total doses of selecoxib and dicrofenac sodium used for pain relief were measured. The circumference of patellar superior border was measured on postoperative days 4,7,14, and 28. The unpaired t-test was used for blood tests as well as for comparisons between the cryotherapy and control groups, and the Mann-Whitney U test was used for the analysis of age, BMI, approach of the surgery, analgesic use, pain scores, the circumference of the thigh and the duration of hospital stay. [Results]. There were no significant differences in age (p = 0.605), BMI (p = 0.790), approach of the surgery (p = 0.572), duration of the surgery (p = 0.117), blood loss during surgery (p = 0.739), or hospital stay (p = 0.169) between the cryotherapy and the control. There were no significant differences between the 2 groups in CK, CRP levels, or pain scores. However, Hb levels measured postoperatively in only day 4 and the total dose of selecoxib was used for pain relief were significantly lower for the cryotherapy group than for the control group, respectively (p = 0.028, p = 0.003), and the total dose of dicrofenac sodium was tend to be significant lower for the cryotherapy group. (p = 0.070). The circumference of patellar superior border measured postoperatively in only day 4 was significantly lower for the cryotherapy group than for the control group (p = 0.010). No complications such as skin problems or neuroparalysis were observed. [Discussion]. This study found the reduction of blood loss, swelling on the patellar superior border, and the total dose of selecoxib for the patients undergoing cryotherapy. However, the pain-relief efficacy of postoperative cryotherapy has not been recognized. Postoperative continuous cryotheraapy is a simple, noninvasive, and effective approach for the reduction of blood loss and swelling following THA

Introduction. Proper positioning of the baseplate and optimal screw placement are necessary to avoid loosening or failure of the glenoid component in RTSA. Several in vitro and anatomic studies have documented ideal number, size, length and angulation of baseplate screws. However, such fixation can often be tenuous, as the anatomy of scapula bone varies. Furthermore, it can be difficult to identify regions with the best bone stock intraoperatively even though surgeons have an understanding of scapular anatomy with potential screw trajectories in mind. This often leads to variable screw lengths and angulations in the clinical setting. The purpose of this study was to measure optimal screw lengths and angles to reach ideal regions in cadaveric scapulae and to compare the clinical experiences of three surgeons with each other and against a cadaveric model with screw lengths and angulations. Materials and Methods. Seven cadaveric scapulae were used as the template for optimal screw angulation and length for baseplate implantation. Total 21 cases (seven cases of each 3 surgeons) of reverse total shoulder arthroplasty using the Aequalis®-Reversed shoulder prosthesis (Tornier, France) were included. Measurement of screw angulation was done on the AP and axillary views to account for the superior-inferior and the antero-posterior angulations, respectively. The screw lengths used on each scapula was recorded prior to insertion in cadavers and retrieved from the operative records in clinical cases. Screws directed anteriorly and superiorly were recorded as positive values while posteriorly and inferiorly directed screws were designated negative values. The significant differences in degrees of screw angulation and screw lengths among the 3 surgeon groups were calculated using the ANOVA, with the p value at 0.05. The Mann-Whitney U test was performed to evaluate the cadaver group against the surgeon groups. Results. In cadaveric specimens, the averages of the screw lengths used were 29.4 mm (anterior screw), 15.4 mm (posterior screw), 36.0 mm (superior screw), and 46.70 mm (inferior screw). The anterior screw was directed 6.9° inferior and 7.5° degrees posterior in reference to the central peg. The posterior screw direction was inferior (−5.0°) and posterior (−1.7°); Superior screw was directed superiorly (30.1°) and anteriorly (22.2°), while the inferior screw was aimed inferiorly (−15.3°) and posteriorly (−8.3°). In clinical cases, the differences in screw length among the 3 surgeon groups were not statistically significant. There was no significant difference in screw angulation among the 3 surgeons except posterior screw. Comparing cadaveric specimens from the clinical cases, the anterior screws were shorter and directed more superiorly and anteriorly in the patients, and the superior and inferior screws were directed less superiorly and inferiorly in the patient. Conclusion. We concluded that more vertical screw placement of the superior and inferior screws is necessary to obtain the ideal baseplate fixation

The purpose of the present investigation was to evaluate muscle damage one year after anterior minimally invasive THA by MRI and to compare these findings with MRI investigations performed in asymptomatic patients one year after THA using a conventional direct lateral approach. Institutional review board approved this study and patients gave signed informed consent. The minimally invasive group consisted of a consecutive series of 25 patients 1 year after anterior minimally invasive THA. The historic control group consisted of a consecutive series of 25 asymptomatic patients (no pain, no limb, full abduction strength) 1 year after conventional THA. Excluded were patients having prior hip surgery or suffering lumbar spine pathology. Tendon defects and degenerations within the insertion of the Gluteus medius and minimus muscles as well as fatty atrophy within these muscles were recorded according to the protocol of Pfirrmann et al. A Mann-Whitney U Test, two sided t-test and Chi-square test were used for appropriate comparison of quantitative and qualitative variables, respectively. In terms of gender, age, BMI and side no significant differences were shown between the two groups. When compared to the conventional group, tendon defects, diameter changes and signal alterations of the Gluteus medius and minimus insertion were significantly less frequent in the minimally invasive group (p= 0.001–0.03). Fatty atrophy within the gluteus minimus and gluteus medius musculature was significantly less in the minimally-invasive group (p=0.001–0.04). In terms of structural damage to muscles and tendons of the hip abductors, the anterior minimally invasive approach proves to be less invasive than the direct lateral approach

Introduction: The effect of facet tropism on the development of lumbar disc diseases has been investigated but is still controversy; moreover, there has been no study to be done on far lateral lumbar disc herniation (LDH). In the current study, the authors attempted to determine the differences of the degree of facet tropism and the degree of disc degeneration between far lateral and posterolateral LDHs. In addition, the effect of the difference of degree of facet tropism and the degree of disc degeneration on the development of far lateral LDH was investigated compared with posterolateral LDH. Methods: 38 LDHs (far lateral, n = 19; posterolateral, n = 19) who underwent posterior open discectomy or paraspinal approach were included in this study. The mean age was 52.3 years in far lateral LDH and 45.3 years in posterolateral LDH. The degrees of facet tropism and disc degeneration were measured at herniated disc level using MRI, and compared for the two different types of LDHs. Mann-Whitney U test and Spearman test were used for analysis. Results: There were significant statistical differences in the degree of facet tropism and the degree of disc degeneration. There was no significant correlation between the degree of facet tropism and the degree of disc degeneration in far lateral LDH. Discussion: The current study suggests that the differences of the degree of facet tropism and the degree of disc degeneration might be considered as the key factors to determine the development of far lateral LDH compared with posterolateral LDH

Objective: To evaluate a new system of prescribing and administering IV morphine introduced by our Paediatric Pain Service for paediatric patients undergoing corrective spinal surgery. Design: An audit of post-operative pain management was conducted retrospectively on patients who had undergone scoliosis correction between November 1999 and September 2001. Subjects: Sixteen patients between the ages of nine and seventeen years who had undergone spinal scoliosis correction during the study period were evaluated. Outcome measures: The average post-operative IV morphine consumption, the pain and sedation scores, the incidence of side effects, and the use of adjuvant analgesics were analysed. Statistical evaluation was carried out using the Student’s T test and the Mann-Whitney U test. Results: Morphine consumption was significantly higher in the first post-operative twenty-four hour period, as well as in the over 13 year-old age group. There was no significant difference in morphine consumption between genders or ASA physical status. The pain scores were significantly higher in males compared to females on day one post-operatively. There was no statistical difference in sedation scores. Conclusions: An audit of the post-operative pain regimen for patients undergoing spinal scoliosis correction provided by the Paediatric Pain Management Service has shown that it is both efficient and safe. We recommend an anticipated pain management pathway of recovery following spinal fusion for idiopathic adolescent scoliosis

Introduction: It was hypothesized that ipsilateral upper extremity loading will decrease hip abductor activity by decreasing the adductor moment and thus relieving the symptoms of patients with gluteus medius tendinitis. The aim of the study was to test the hypothesis with a electrophysiological and clinical study. Materials and Methods: Seven voluntary men with no hip complaint were included in the electrophysiological phase of the study. The motor unit activities of the gluteus medius were measured for each subject during the consecutive loading of the ipsilateral, contralateral and both upper extremities, with 2, 3, 5 and 7 kilograms. Seventeen patients (age range 34–67) with acute symptoms of gluteus medius tendinitis were included in the clinical phase of the study. Dynamic VAS scores were recorded for each patient during gait with consecutive loading of the ipsilateral, contralateral and both upper extremities, for each 2, 3, 5 and 7 kilograms. Wilcoxon and Mann-Whitney U tests were used in statistical analysis. The p values below 0.05 were considered significant. Results: In the electrophysiologic study the ipsilateral upper extremity loading with 7 kg in 4 patients, and with 5 and 7 kilograms in 2 patients resulted in a motor unit activities with moderate interference, while motor unit activities with full interference were recorded during all other loadings. The motor unit activity interference scores recorded during the ipsilateral upper extremity loading with 5 kilograms of above, were significantly less then all other loading combinations (p < 0.05). In the clinical study, the ipsilateral upper extremity loading with 3 kilograms and above resulted in significantly less VAS scores then the other loading combinations (p< 0.05). Conclusion: Ipsilateral upper extremity loading decrease the motor unit activity of the ipsilateral gluteus medius muscle and relieves the symptoms of the patients with gluteus medius tendinitis. Ipsilateral upper extremity loading can lead up to new strategies in the rehabilitation of gluteus medius tendinitis
