We describe a new technique of reconstruction of the deficient acetabulum in cementless total hip arthroplasty. The outer iliac table just above the deficient acetabulum is osteotomised and slid downwards. We have termed this an iliac sliding graft. Between October 1997 and November 2001, cementless total hip arthroplasty with an iliac sliding graft was performed on 19 patients (19 hips) with acetabular dysplasia. The mean follow-up was 3.4 years (2 to 6). The mean pre-operative Harris hip score was 45.1 which improved significantly to 85.3 at the time of the final follow-up. No patient had post-operative abductor dysfunction. Incorporation of the graft was seen after two to three months in all patients. Resorption of the graft and radiolucencies were infrequent. This technique is a useful alternative to femoral head autografting when the patientā€™s own femoral head cannot be used

To devise an operative approach to the management of acute posterior fracture-dislocation of the shoulder which restores or retains normal proximal humeral anatomy and allows the early restoration of a complete, stable range of motion. Since 1996 we have treated four male patients (five shoulders) aged between 19 and 54 years at the time of first dislocation with autogenous iliac grafting of the anterior humeral head defect for acute and acuteon-chronic posterior dislocation of the shoulder. Two patients had epilepsy: one of these patients had bilateral dislocations. Two patients had motorbike RTAs. The deltopectoral approach with vertical division of the subscapularis tendon was used in all cases. The defects comprised 20ā€“ 25% of the volume of the humeral head at the equator after preparation for grafting. Grafts were fixed with compression screws. The subscapularis tendon was repaired anatomically. Active-assisted rehabilitation was started immediately, restricting external rotation to the neutral position for six weeks, thereafter allowing full rotation and elevation as comfort allowed. The patient with bilateral dislocations died of unrelated causes 18 months after surgery. He was reported to have had no further dislocations, complete pain free functional use of both shoulders and no complications of the procedure. The remaining three patients were reviewed at a minimum of 20 months after surgery (average 35 months). All grafts had incorporated. There was no graft collapse or boundary arthrosis. The absolute Constant scores were 85.1, 90.9, and 89.2; the subjective shoulder scores were 98%, 90%, and 99%; the Oxford rating scale for pain scores were 14 out of 60, 13 out of 60, and 14 out of 60; and the Oxford rating scale for instability scores were 14 out of 60, 15 out of 60 and 15 out of 60. There were no redislocations, or complications of the procedures. Posterior stability appears more dependant on surface arc of contact than on capsular integrity, in contrast to the anteriorly unstable shoulder. Restoration of the articular surface arc of contact by segmental autogenous grafting retains normal humeral anatomy, allows normal motion with excellent cuff function, and a return to normal daily activities. The procedure has been shown to be safe at a minimum of 20 months


There is a high risk of the development of avascular necrosis of the femoral head and nonunion after the treatment of displaced subcapital fractures of the femoral neck in patients aged < 50 years. We retrospectively analysed the results following fixation with two cannulated compression screws and a vascularised iliac bone graft. We treated 18Ā women and 16 men with a mean age of 38.5 years (20 to 50) whose treatment included the use of an iliac bone graft based on the ascending branch of lateral femoral circumflex artery. There were 20 Garden grade III and 14 grade IV fractures. Clinical and radiological outcomes were evaluated. The mean follow-up was 5.4 years (2 to 10). In 30 hips (88%) union was achieved at a mean of 4.4 months (4 to 6). Nonunion occurred in four hips (12%) and these patients had a mean age of 46.5 years (42 to 50) and underwent revision to a hip replacement six months after operation. The time to union was dependent on age with younger patients achieving earlier union (p < 0.001). According to the Harris hip score which was available for 27 of the 30 hips with satisfactory union, excellent results were obtained in 15 (score ā‰„ 90 points), fair in ten (score 80 to 90 points), and poor in two hips (ā‰¤ 80 points). One patient aged 48 years developed avascular necrosis of femoral head six years after operation and underwent total hip replacement.

The management of displaced subcapital fractures of the femoral neck, in patients aged < Ā 50 years, with two cannulated compression screws and an iliac bone graft based on the ascending branch of lateral femoral circumflex artery, gives satisfactory results with a low rate of complication including avascular necrosis and nonunion.

Purpose: The retrospective evaluation of long-term results after reconstructive radial osteotomy for mal-united distal radius fractures.

Material-Method: Twenty-eight patients (21 male and 7 female, average aged 46 years) with 23 dorsal and 5 palmar angulated malunited distal radius fractures were operatively treated during 1994ā€“2002 in our department. The main indications were pain and functional impairment. Dorsal or palmar approach was used in proportion to the site of angulation. The preoperative average radial inclination, radial length and volar or dorsal tilt were 13.5 degrees, 6.3 mm and 23.5 degrees respectively. An open wedge radial osteotomy followed by interposition of trapezoidal iliac crest bone graft and fixation with plate ands crews was performed in all patients four months at least after the initial surgery. An ulnar leveling procedure was considered necessary in 2 patients.

Results: All patients were available in the last follow up evaluation (mean 3.7 years). The functional result according to Mayo wrist score was rated as very good in 15 patients, good in 7 and poor in 6. The average improvement in radial inclination was 14 degrees, in radial length 6.5 mm and in volar or dorsal tilt 21 degrees. The complication rate was 22.7%, including 2 material failures, 1 extensor pollicis longus rupture, 1 nonunion and 3 recurrences of the deformity.

Conclusion: Surgical reconstruction for malunion is technically demanding and may not completely restore the anatomy. Patient satisfaction, however, in terms of increased function, decreased pain and decreased deformity is sufficient high to warrant reconstructive treatment.

Aims. The purpose of this systematic review was to determine the rates of union for vascularized versus non-vascularized grafting techniques in the operative management of scaphoid nonunion. Secondary aims were to determine the effect of the fixation techniques used, the source of grafting, as well as the influence of fracture location (proximal pole) and avascular necrosis (AVN). Methods. A search of PubMed, MEDLINE, and Embase was performed in June 2021 using the Preferred Reporting Items for Systematic Review and Meta-Analyses statement and registered using the PROSPERO International prospective register of systematic reviews. The primary outcome was union rate. Results. There were 78 studies that met the inclusion criteria with a total of 7,671 patients (87.8% male, 12.2% female). The mean age was 27.9 years (SD 3.8) and the mean follow-up was 30.9 months (SD 25.9). The mean union rate was 88.7% (95% confidence interval (CI) 85.0 to 92.5) for non-vascularized grafts versus 87.5% (95% CI 82.8 to 92.2) for vascularized grafts (p = 0.685). Pooled analysis of trial data alone found a mean union rate of 82.4% (95% CI 66.9% to 97.9%) for non-vascularized grafts and 89.4% (95% CI 84.1% to 94.7%) for vascularized grafts (p = 0.780). No significant difference was observed in union rates between any of the fixation techniques used in the studies (p = 0.502). Distal radius and iliac crest graft source had comparable mean union rates (86.9% (95% CI 83.1 to 90.7) vs 87.6% (95% CI 82.2 to 92.9); p = 0.841). Studies that excluded patients with both proximal pole fractures and AVN (n = 14) had a mean union rate of 96.5% (95% CI 94.2 to 98.9) that was significantly greater than the mean union rate of 86.8% (95% CI 83.2 to 90.4) observed in the remaining studies (p < 0.001). Conclusion. Current evidence suggests vascularized bone grafting does not yield significantly superior results to non-vascularized grafting in scaphoid nonunion management. However, potential selection bias lessens the certainty of these findings. The fixation type or source of the graft used was not found to influence union rates either. Sufficiently designed and powered prospective randomized controlled trials in this area are needed. Cite this article: Bone Joint JĀ 2022;104-B(5):549ā€“558

Aims. We hypothesized that there is no difference in the clinical and radiological outcomes using local bone graft versus iliac graft for subtalar distraction arthrodesis in patients with calcaneal malunion. In addition, using local bone graft negates the donor site morbidity. Patients and Methods. We prospectively studied 28 calcaneal malunion patients (the study group) who were managed by subtalar distraction arthrodesis using local calcaneal bone graft. The study group included 16 male and 12 female patients. The median age was 37.5 years (interquartile range (IQR) 29 to 43). The outcome of the study group was compared with a control group of ten patients previously managed by subtalar distraction arthrodesis using iliac bone graft. The control group included six male and four female patients. The median age was 41.5 years (IQR 36 to 44). Results. The mean American Orthopaedic Foot & Ankle Society (AOFAS) hindfoot score improved significantly in the study and the control groups (p < 0.001). Fusion was achieved in 27 patients in the study group at a median time of 13 weeks (IQR 12 to 14), while all the patients in the control group achieved fusion at a mean time of 13.2 weeks (11 to 15). The mean talocalcaneal height and talar declination angle improved significantly in both the study and the control groups (p < 0.001). There was no significant difference between both groups concerning the preoperative or the postoperative clinical and radiological measurements. Donor site morbidity was reported in four out of ten patients in the control group. Conclusion. Local calcaneal bone graft can successfully be used to achieve subtalar distraction arthrodesis with appropriate correction of alignment and calcaneal malunion. We recommend using local instead of iliac bone graft as it gave comparable results and avoids the possibility of donor site morbidity. Cite this article: Bone Joint J 2019;101-B:596ā€“602

Background: Bone morphogenetic proteins (BMPs) induce new bone in patients with bone defects and at extraskeletal sites in animals. Standard treatment for symptomatic scaphoid non-unions is bone graft with or without internal fixation by a screw or wires. We tested the ability of human recombinant osteogenic protein-1 (OP-1, BMP-7) with compressed autologous or allogeneic bone graft to accelerate the healing of scaphoid non-union. Study Design: Randomized and controlled pilot study in 17 patients with a scaphoid nonunion. Methods: Patients were randomly assigned to one of three groups: (1) Autologous iliac graft (n=6), (2) Autologous iliac graft + OP-1 (n=6) and (3) Allogeneic iliac graft + OP-1 (n=5). Radiographic, scintigraphic and clinical outcomes were assessed throughout the follow-up period of 24 months. Results: OP-1 improved the performance of both autologous and allogeneic bone implants. Three dimensional helical CT scans and scintigraphy showed that the pre-existing sclerotic bone within proximal scaphoid poles was mainly replaced in OP-1 treated patients with well vascularized new bone. Addition of OP-1 to allogeneic bone implant equalized the clinical outcome with the autologous graft procedure and enabled circumventing the second donor graft harvest procedure resulting in less blood loss, shorter anesthesia and no pain at the donor side. Conclusion: This is the first evidence that a recombinant human BMP accelerates scaphoid bone non-union repair and resorption of sclerotic bone in this specific microenvironment. Clinical Relevance: OP-1 might be successfully used in healing of scaphoid non-union

Purpose of the study: Although the iliac autograft is the gold standard for single-level intervertebral fusion, complications and morbidity related to autologous graft harvesting from the iliac crest remain a point of concern. Bone morphogenic protein (BMP) has proven advantages for fusion of the intersomatic and posterolateral graft. This study compared the efficacy and tolerance of OP-1 compared with an autologous graft in patients with symptomatic spondylolisthesis. This study reports the preliminary results of a prospective randomised controlled trial comparing OP-1 with an iliac autologous graft for instrumented single-level posterolateral fusion for arthrodesis of grade 1 spondylolisthesis. Material and methods: Lamino-arthrectomy associated with a posteriolateral instrumented arthrodesis with an iliac autologous graft or a mixture of OP-1 and local autologous graft material was performed in 27 patients with spondylolisthesis leading to lumboradiculalgia or neurogenic claudication. The final outcome was time to fusion at one year on the scanner and plain x-rays. The Oswestry score and pain at the harvesting site as well as side effects were also noted. Results: The cohort included 27 patients. Three were excluded from the analysis, leaving 24 patients assessed at one year. The demographic data were comparable for the two groups regarding mean age (64 years versus 69 years for the OP-1 group). At one year, ten radiographically certain fusions were noted in the control group and eight in the OP-1 group. Two nonunions and one doubtful fusion were noted in the control group compared with three doubtful fusions in the OP-1 group. The mean Oswestry score was comparable in the two groups. The mean score in the control group improved from 49.5 to 28.5 compared with 45.9 to 29.7 in the OP-1 group. There was no secondary effect attributable to use of OP-1. There were no cases of systemic toxicity, nor heterotopic calcification or restenosis for the 11 patients in the OP-1 group. Conclusion: A fusion rate of 73% without secondary effects attributable to OP-1 was observed in this preliminary study. This study allows the conclusion that this technique is reliable, safe and, in terms of fusion, a valid alternative to autologous iliac crest graft. The main advantage resulting from the use of OP-1 is to avoid the morbidity linked with harvesting the iliac graft

Purpose: Total ankle arthroplasty remains a difficult procedure. Some patients require revision surgery for arthrodesis. Material and methods: We report nine patients with total ankle arthroplasties mainly implanted for post-traumatic osteoarthritis whose results deteriorated, requiring arthrodesis. One of these patients had rheumatoid arthritis. Revision surgery was performed six months to seven years after arthroplasty. Arthrodesis was required for pain related or not to implant loosening or talar necrosis. One patient developed a major deviation of the hind foot secondary to progressive loosening. One patient developed infection early. An iliac graft was used to fill the bone defect in eight patients. An anterior plate-screw fixation was used for six patients, crossed screws for one, a tibiotalar nail for one, and an external fixator for one (with infection). Results: Eight patients achieved bone healing with good pain relief. The functional result depended on the type of arthrodesis: talocrural alone or extended to the torsion couple. Discussion: The evolution of ankle prostheses toward better bone sparing has allowed, in our experience, for revision arthrodesis under relatively good conditions using an iliac graft. We have not preferred one standard type of fixation but the anterior plate fixation has provided excellent stability

Objectives: To evaluate the clinical and radiological outcome following anterior interbody fusion using a femoral cortical allograft packed in the centre with autogenous iliac graft combined with posterior pedicle fixation. Design: A prospective study of 30 consecutive adult patients diagnosed with lumbar degenerative disc disease. Summary of background data: Various fusion options have been described in the literature with various clinical and radiological outcomes. Narrowing of the disc space is thought to be the primary cause of all the potential secondary spondylotic changes; the ideal corrective surgery would be one that reconstitutes the disc space interval. A hybrid graft consisting of a femoral cortical allograft (FCA) ring packed in the centre with autogenous cancellous iliac graft seems to be an ideal option as one should capitalize on the mechanical strength of the cortical ring and the biological strength of the cancellous autogenous graft. Methods: 15 patients with failed back surgery, 15 patients with primary degenerative disc disease. Mean age was 43 years. All patients had preoperative radiographs, MRI scanning, discography was not used in this study. All femoral allografts were processed by freeze drying and sterilized using ethylene oxide. The protocol for surgery consisted of: . * Left sided retroperitoneal muscle splitting approach. * Anterior hybrid interbody grafting. * Pedicle screw fixation without grafting. Results: There were no complications from using the allograft. Intraoperative and postoperative complications were seen in 3 patients. After a minimum follow up of 2 years good clinical outcome was achieved in 83 % using Ricciardi et al criteria, solid fusion was achieved in 98%. Conclusion: The results in this study continue to support the efficacy of using hybrid interbody grafts to achieve a successful fusion, however there is discrepancy between the radiological and clinical outcome which merits additional investigations

Aims: Study our experience and short term results using a mix of osteoconductive (HA) and osteoinductive (AGF) materials. Methods: From October 2001 until June 2002, we have treated bone defects in 9 patients. Seven male and 2 female. Mean age 10.4 years (range 4ā€“18 years). Mean follow-up: 5.6 months (range 3ā€“9 months). AGF was obtained after autologous blood centrifugation according to blood volume, knowing the patient height and weight (Nadler Score). AGF was obtained through previous concentrate of platelets and red cells, with a further concentration, reducing its volume to 1/3. 10 c.c. of thrombin (500 UI) and HA (500R) were added, just before applying it to the patient. Total surgery time for preparation AGF was 20ā€“30 minutes. Clinical cases treated were: varus osteotomy in Perthes (1 case- 11%); curettage in osteomyelitis (2 cases- 22%); essential cyst, after conventional corticoid treatment failure (2 cases- 22%); forearm pseudoarthrosis (2 cases- 23%) and triple arthodesis by valgus pronated spastic foot (2 cases- 22%). We never use autologous iliac graft with AGF- HA. Results: We have obtained radiological and clinical consolidation in all bone defects after 3ā€“4 months. Radiological success is not clear after 4 months in one of the osteomyelitis cases (12ā€™5%). Conclusions: 1. The iliac graft harvest morbility is about 9.4%- 49%. 2. A 2nd approach is avoided in children and adolescents. 3. With the use of AGF- HA we avoid morbility, diseases transmission, reduced surgery time and offer an alternative to autologous grafting

Purpose: Failure of the glenoid component is the main complication of total shoulder prostheses. When surgical revision is necessary, the surgeon has the option of a new implantation or non-prosthetic plasty (glenoido-plasty). The purpose of the present work was to analyse results obtained with these two techniques in order to propose proper indications. Material and methods: This retrospective study included 16 patients, mean age 62 years at revision surgery. Fialures included loosening of a cemented glenoid implant (n=9) and failure of non-cemented implants (3 defective anchors, 4 unclipped polyethylene inserts). Mean time to revision was 39 months (2ā€“178) after primary implantation. A new glenoid implant was cemented in nine patients (group A). Seven patients (group 2) had glenoidoplasty with an iliac graft in four. Results: A mean follow-up of 37 months (19ā€“73), the Constant score had improved from 18 points before revision to 52 points (+34). Two patients experienced a complication requiring a second revision (infection, instability) and one patient underwent subsequent surgery for biceps tenodesis. For the group with glenoidoplasty with iliac graft, insertion of an inverted prosthesis was achieved during a second operative time. In group 1, the mean Constant score at last-follow-up was 63 points (+45) with the pain score of 11, movement score of 29. In group 2, the mean Constant score was 37 (+19) with pain at 6 and motion at 16. In this group, the mean score was 48 points with a glenoid graft and 21 points with simple implant replacement. Discussion: Revision surgery for a failed glenoid implant remains a difficult procedure but can be effective for pain relief and improved motion score. The small number of patients in this series makes it difficult to perform statistical analysis but the results do point in favour of prosthetic reimplantation when the bone stock is sufficient. For other patients, a graft would be preferable to simple implant removal. This would allow secondary revision if possible

Purpose: Meralgia paraesthetica is usually caused by entrapment of the lateral femoral cutaneous nerve (LFCN) at the inguinal ligament. We present our experience with 114 patients who underwent surgical management for meralgia paraesthetica. Material: We reviewed 114 patients (48 men, 66 women, five bilateral cases) who underwent surgery for meralgia paraesthetica between 1987 and 1999; local anaesthesia was used for neurolysis in most cases. We identified five aetiologies: idiopathic (n=69, three bilateral), abdominal surgery (n=19), iliac graft harvesting (n=12, one bilateral), hip surgery (n=7), trauma (n=7, one bilateral). Methods: We analysed outcome at more than two years follow-up for the entire series and by aetiology using a standard 12-point evaluation scale accounting for residual pain, sensorial disorders, and patient satisfaction. Results: The overall results were good, mean score 9/12 (range 1ā€“12). Ninety-two patients were very satisfied or satisfied. Among the 27 patients who were not satisfied, five developed recurrence. Mean time to full pain relief was 70 days (range 1 ā€“ 364 days). Recovery of thigh sensitivity was noted at 128 days (range 1 ā€“ 364). Discussion: The essential criteria of poor prognosis were duration of the meralgia before surgery and its aetiology. Neurolysis of an LFCN injured by trauma or iliac graft harvesting provided less satisfactory results (scores 7 and 6 respectively) than for idopathic meralgia paraesthetica or abdominal-surgery injury (scores 9 and 10 respectively). Eight of the neurolysis procedures in this series did not provide satisfactory results (score 5). Conclusion: Neurolysis appears to be the surgical treatment of choice for mearlgia paraesthetica. In skilled hands, neurolysis can be performed under local anaesthesia, although certain difficulties can be encountered: obesity, modified anatomy due to prior operations, nerve variability (frequent). Knowledge of these different elements is essential not only to achieve neurolysis but also prevent iatrogenic injury

Purpose: Scaphotrapezotrapezoidal (STT) pain is common but often asymptomatic. Medical treatment may be proposed if symptoms become bothersome. In case of failure, several surgical solutions may be proposed. The purpose of this work was to assess outcome in a series of eleven STT arthrodeses performed for isolated STT osteoarthritis. Material and methods: Ten patients, three men and seven women (11 hands), mean age 63 years, developed STT osteoarthritis which was treated by arthrodesis. All patients experienced pain for daily life activities and had diminished wrist movement. According to the Crosby radiographic classification, three were one grade I, four grade II, and five grade II with carpal misalignmen t. One patient had chondrocalcinosis and six had tendinitis of the flexor carpi radialis. The anterior approach was used for three patients and the lateral approach for seven. Nine patients had an iliac graft to fill bony defects resulting from anterior wear. Pin fixation was used in six cases and staple fixation in five. An antebrachiopalmar cast was maintained for at least six weeks after surgery. Results: At mean follow-up of 62 months, the pain score improved in all patients (p=0.05). There was no significant difference in motion, excepting decreased wrist extension (12Ā°, p=0.03). Grasp and pinch force were not modified by STT arthrodesis. All patients were able to resume their former recreational and occupational activities. There was no worsening of the five cases with intracarpial deaxation. Four patients developed non-union (three pin fixations, one staple fixation), which was symptomatic in only two. These patients underwent successful revision using the same fixation technique. There were no complications. Discussion: Like other series reported in the literature, our series of STT arthrodeses demonstrated effective pain relief. However, unlike former work, we were unable to obtain a significant reduction in the radial inclination force, or radio-carpal conflict. The other therapeutic option is resection of the distal pole of the scaphoid which provides more rapid clinical results but which leads to inevitable misalignment of the carpus. Conclusion: We have decided to retain STT arthrodesis with iliac graft for patients with STT osteoarthritis, especially for young patients

Purpose: Necrosis of the navicular bone, described by MĆ¼ller then Weiss in 1927, is an uncommon finding, unlike talonavicular degeneration which is a rather frequent complication of talipes planovalgus. Between 1985 and 2000, we cared for 25 patients with this condition. The purpose of this retrospective analysis was to describe the clinical and radiological presentation and attempt to reconstruct its natural history with the aim of determining therapeutic indications. Material and methods: We analysed 25 cases of navicular bone necrosis observed in 14 women and 3 men (eight bilateral cases). Mean age of the patients was 39 years (range 16ā€“59). The diagnosis of necrosis was established on the basis of structural alterations (densification, bone defects) and in the more advanced cases, flattening and ā€œexpulsionā€ of the navicular bone. We looked for clinical signs and described the radiological aspect of the necrotic zone. A computed tomography was available in 14 cases and magnetic resonance imaging in the five most recent cases. Results: Pain was the major sign in all cases. One-third of the cases occurred in a foot with prior planovalgus. History taking revealed elements suggestive of an aetiology in three cases: probable Kƶhler-Mouchet disease in a 16-year-old boy, sickle cell disease in a 35-year-old man, and prolonged walking with signs suggesting stress fracture in a 40-year-old woman. In the other 19 cases (11 women and 1 man, 7 bilateral cases), necrosis was considered idiopathic. Radiologically, we used the Ficat classification (described for hips): stage 0 with normal x-ray and strong uptake on scintigram (n=1), stage 1 with a normally shaped navicular bone but condensation or bone defect, stage 2 with modification of the shape of the bone without signs of degeneration, stage 3 where changes in the shape of the bone are associated with narrowing of the talonavicular then cuneonavicular space. Computed tomography included frontal and horizontal slices as well as lateral reconstructions indispensable to assess the posterior part of the interarticular spaces. Treatment was surgical in 12 cases and medical in 13. Well tolerated forms were treated with plantar ortheses with regular surveillance. Surgical procedures included triple arthrodesis (early in our experience), mediotarsal arthrodesis (n=2), talonavicular arthrodesis (n=7) and talocuneate arthrodesis with replacement of the scaphoid by an iliac graft (n=2). The natural course of necrosis was studied in the cases without surgery. The first sign was medial mediotarsal pain. At this stage scin-tigraphy or MRI was required for positive diagnosis. At stage 0 condensation of the navicular bone, confirmed by computed tomography, preceded bone flattening then expulsion upwardly and medially, sometimes with fragmentation and onset of talonavicular degeneration. Cuneonavicular degeneration appeared to occur later (except in one case). Long-term results of surgery were good with pain relief and renewed activity. Discussion: The clinical presentation initially described as MĆ¼ller-Weiss disease or scaphoiditis, which concerns a bilateral condition generally occurring after trauma and sometimes with a favouring factor (alcoholism, osteoporosis), appears somewhat different from our description. Mechanical factors predominated in our patients and the aetiologies were quite similar to those observed in Kienbƶck syndrome. Excessive pressure on the navicular bone, which leads to avascular necrosis, flattening, and expulsion, is undoubtedly the essential cause of this condition. It is well tolerated in some individuals and can lead to spontaneous fusion. In this situation, treatment can be limited to surveillance or orthopaedic care. If the functional impact is important, surgical treatment can be proposed, generally limited to talonavicular arthrodesis. If the navicular bone is sclerosed and flat, the remaining fragment can be replaced by an iliac graft to achieve talocuneate fusion. Conclusion: Necrosis of the navicular bone appears to be less uncommon than in the classical description, particularly in black women aged 25ā€“50 years. A more precise study of favouring anatomic factors (length of the medial ray, size of the talar neck, depression of the medial arch) could provide further information concerning the aetiology which appears to be similar to that of Kienbƶck disease

Aims

Reverse total shoulder arthroplasty (rTSA) can be used in complex cases when the glenoid requires reconstruction. In this study, a baseplate with composite bone autograft and a central trabecular titanium peg was implanted, and its migration was assessed for two years postoperatively using radiostereometric analysis (RSA).

Methods

A total of 14 patients who underwent a rTSA with an autograft consented to participate. Of these, 11 had a primary rTSA using humeral head autograft and three had a revision rTSA with autograft harvested from the iliac crest. The mean age of the patients was 66 years (39 to 81). Tantalum beads were implanted in the scapula around the glenoid. RSA imaging (stereographic radiographs) was undertaken immediately postoperatively and at three, six, 12, and 24 months. Analysis was completed using model-based RSA software. Outcomes were collected preoperatively and at two years postoperatively, including the Oxford Shoulder Score, the American Shoulder and Elbow Score, and a visual analogue score for pain. A Constant score was also obtained for the assessment of strength and range of motion.

To compare the clinical outcomes of instrumented fusion for single level degenerative spondylolisthesis with local bone versus iliac crest bone graft. Fifty patients (32 female, 18 males) operated on by the author over a 3 year period were reviewed. All cases had a single level decompression and instrumented fusion for a degenerative spondylolisthesis. 25 patients had iliac crest graft and 25 had morcelised local bone graft. Patients were followed up for 6 months. Pre and postoperative visual analogue pain scores and Roland disability scores were recorded. Inpatient notes were reviewed for duration of surgery and duration of stay. There was no difference in age, sex and severity of pre operative symptoms between the two groups. There was no significant difference in improvement in Roland score between the two groups but pain scores were lower in the local graft group although this was not statistically significant. Duration of surgery (140 vs 175min) and hospital stay (4.3 vs 5.1 days) were lower in the local bone graft group.6 patients in the iliac crest graft group complained of donor site pain vs none in the local graft group at 6 months. Usage of morcelised local bone graft resulted in clinical outcomes comparable to iliac crest bone graft in patients undergoing decompression and fusion for a single level degenerative spondylolisthesis. Duration of surgery, hospital stay and donor site pain are reduced when local bone was utilised

1. A forty-nine-year-old man had a chondrosarcoma arising from the body of the seventh thoracic vertebra. The tumour protruded into the mediastinum and also into the spinal canal where it displaced the spinal cord. 2. At operation all the seventh thoracic vertebra and parts of the sixth and eighth were removed together with the tumour. The thoracic spine was reconstructed by inserting two iliac bone-blocks between the cut bodies of the sixth and eighth vertebrae and by wiring two strong "A. O." plates to the transverse processes of the third to the sixth and the eighth to the tenth vertebrae. 3. The patient was nursed in a plaster-of-Paris bed for three and a half months. 4. One year and three months after operation, the patient was walking and well, with no signs of recurrence or metastasis. Radiographs showed that a block-vertebra had been created from the iliac grafts and the two cut vertebrae

Background. Medial opening-wedge high tibial osteotomy is one of the common surgical procedures in treatment of knee deformities. Many methods have been proposed to fill the medial side osseous gap. The results of using allograft as void filler compared to iliac crest autograft has not been subject to a randomized clinical trial. The purpose of this study was to examine the results of medial opening-wedge high tibial osteotomy using iliac crest allograft as compared to iliac crest autograft. Materials & Methods. Forty-six patients with genovarum deformity were enrolled based on specific inclusion and exclusion criteria and were randomly assigned into two groups. Medial opening-wedge high tibial osteotomy was done using iliac crest allograft (23 patients) or autograft (23 patients) and the osteotomy site was internally fixed using proximal tibial T-plate. All patients were followed-up to 12 months after surgery. Anatomical indices of proximal tibia, complications of treatment, and functional outcome (using WOMAC osteoarthritis index) were assessed for both groups. Results. The amount of correction (degrees), recurrence of the deformity and loss of correction and time to clinical or radiologic union were similar in both groups with no statistically significant difference. Duration of operation was significantly less in allograft group (66.6Ā±3.6 versus 52.9Ā±5.3 minutes, p<0.001). Incidence of surgical site infection did not significantly differ in two groups. No nonunion or delayed union was encountered in either group. Some patients reported more intense postoperative pain in iliac graft harvest site than tibial osteotomy site. Patients in both groups had statistically significant improvement in WOMAC index postoperatively (with no statistically significant difference between groups). Conclusions. According to the results of this study, iliac crest allograft may be safely used in medial opening-wedge high tibial osteotomy with comparable efficacy to iliac crest autograft in patients who do not accept the morbidity of autograft harvest

To provide short- term follow-up data on the surgical success and patient outcome following early anterior cervical fusion in this particular type of injury. A prospective study of 10 consecutive patients. Stage I compressive extension injury of the cervical spine, as described by Allen and Ferguson, is not always a stable injury. The combined unilateral failure of the posterior structures under compression together with failure of the anterior structure under tension will lead to a rotationally unstable segment. Various treatment options are available including halo vest immobilization, posterior stabilization with plating and anterior fusion and plating. 10 consecutive patients diagnosed with stage I compressive extension injury (fracture subluxation of the cervical spine). All subjects presented with a neurological deficit and vertebral subluxation. All patients were investigated with CT scan of the involved segment; in addition 2 patients had MRI scans. The surgical protocol consisted of early reduction followed by anterior cervical fusion using a tricortical iliac graft, and stabilization, using locking plate fixation. Follow-up was by radiographs and clinical examination. Intraoperative assessment revealed disc injury in all patients. Anatomical realignment was achieved together with a solid fusion in all of the patients. All patients showed improvement in the neurological deficit. One patient remains with some residual weakness in his triceps and another patient required removal of a prominent screw. Early anterior fusion and plating for this type of injury is a safe procedure

Introduction: Internal fixation of distal radius fractures with dorsal plates (when needed) comprise two potential problems: efficacy of stabilization and the high frequency of tendonitis which forced us to early removal of the hardware. Our purpose is to study the efficacy of the method of stabilization with 2 plates (2mm) the central and radial columns of distal radius according to the technique presented by Regazzoni (1993). Material and Methods: Eight patients (5 males and 3 females), average 35 years old (range, 20ā€“52 years) were treated with comminuted intraarticular fracture type C (6 patients) or malunion of distal radius (2 patients). A combined approach was used in 4 patients and only dorsal approach in the rest 4 patients. In all patients with recent comminuted fractures a supplementary fixation method with allograft were used. In patients with mal-united fractures the technique with two plates together with iliac graft were applied. Results: Results estimated after mean follow-up of 14 months (range 9ā€“18 months) according to clinical (pain, range of motion, and grip strength) and radiological (articular congruency, radial height, radial inclination and palmar tilt) criteria. Six patients revealed excellent or good result although two patients with loss of reduction needed a second operation. Conclusion: We concluded, that when the technique with 2 plates was applied correctly the stability of fixation enhanced because the detrimental rotational forces were cancelled while the frequency of tendon irritation decreased

Purpose: Minimally invasive video-assisted thoracotomy can be proposed for potentially unstable fractures of the thoracolumbar junction with rupture of the anterior column after satisfactory posterior reduction and osteo-synthesis. Long-term results are improved in terms of graft quality and stability of the postoperative angular gain. Material and methods: Ninety-one patients, mean age 36 years, with spinal fractures involving T12 to L2 underwent video-assisted mini-thoracotomy for arthrodesis as a complementary procedure after posterior reduction and osteosynthesis without bone graft. The left approach was preferred over the right due to the lower risk of bleeding. A massive tricortical anterolateral iliac graft was inserted in the intersomatic space. Perfect stability was achieved with the MIASPAS system which provides an exact measure of the intersomatic space and the bone graft. Results: Follow-up was five years for the first patients and ten months for those operated on after January 2003. At last follow-up, functional outcome (Stauffer-Coventry score) was excellent or good in 52% and 41% of the patients respectively. Outcome was poor in 7%. Postoperative radiograms demonstrated 100% graft fusion which was confirmed by tomography. There was no loss of angular gain. Discussion: This novel approach provides a means of preserving the posterior instrumentation and achieving anterior fusion in the zone of maximal stress. Because of the relative facility of the technique together with the operative time and the relatively low risk of postoperative complications, surgical indications for complementary intersomatic arthrodesis can be revisited. Anterior fusion is, in our opinion, the only sure means of maintaining long-term correction. The ability to access the disc, generally without metameric hemostasis, eliminates the risk of vascular cord lesions. Conclusion: This thoracoscopic technique for anterior fusion after a short posterior osteosyntheis without graft will greatly modify indications for complementary anterior fusion for the treatment of unstable thoracolumbar fractures

Forefoot reconstructive surgery can be complex and intricate, and even though performed by orthopaedic surgeons, it can be delicate, too. Despite the most ingenious techniques, patients routinely walk (stomp) all over this work, and the resulting forces applied to the foot have been extensively studied in gait analysis laboratories. But the everyday clinical challenge is how to employ durable reconstructive techniques, and how to salvage these case when they fail?. Hallux valgus surgery is replete with complications of malunion, non-union, over-correction and recurrence. Salvage often requires a revision of the patientā€™s expectations in addition to another surgery. First metatarso-phalangeal joint (MTP) arthrodesis, which has been demonstrated to have excellent functional outcomes, including return to sports activities, is an excellent salvage technique. Failed first MTP arthroplasty leaves a large bone defect, both in the metatarsal and phalanx. Salvage by arthrodesis requires bone grafting, rigid internal fixation, and long healing times. Tricortical iliac graft can be used to enhance restoration of length. Associated meta-tarsalgia may persist due to shortening of the first ray. Lesser metatarsal salvage sometimes can be accomplished with distal metaphyseal osteotomy. Some cases previously treated with a pan-metatarsal head resection can now be successfully salvaged by using these osteotomies to shorten multiple metatarsals. Meticulous technique is an advantage. First MTP arthrodesis itself can be complicated. One uncommonly recognized problem is the painful pressure under the residual sesamoid bone following an otherwise successful fusion. This is caused by arthritic enlargement of the sesamoid, and should be anticipate, and prevented by sesamoidectomy. Surgical technique for obtaining ideal position of the hallux will be presented. The distinction between true recurrence of interdigital (Mortonā€™s) neuroma, and metatarsalgia can be subtle. The cause of failed neuroma surgery is as frequently an error in patient selection as it is failure of surgical technique. Case presentations will be employed to illustrate forefoot salvage principles

Introduction and Aims: Chronic pyogenic osteomyelitis, with pathological fractures, sequestra and subsequent bone defects, is still a major problem in developing countries. The treatment is challenging. Unhealthy skin with discharging sinuses make routine grafting procedures difficult. The aim of this paper is to describe the methods used to treat resulting defects. Method: Thirty-four children, aged one to 12 years, treated between 1991 and 2002, were reviewed. The tibia was involved in 24 children, femur (five), radius (four), and ulna (one). Twenty-nine children had sequestrectomy and debridement. Five children presented with established bone defects. Bone defects measured 1ā€“20cm. Four methods of grafting were used. Autogenous onlay grafts were used in defects < 2cm in seven children. The Papineau technique was used in cavitating defects in two, bicortical segmented iliac crest grafts (1cm x 1.5cm) threaded over a Kirschner wire were used in 15 defects (< 10cm) and bone transfer was performed in six children ā€“ proximal fibula to tibia (five) and radius to ulna (one). Results: Three children required repeat grafting, one for fracture and three for non-union. All the remaining grafts incorporated by three to six months. Distal procedures including tibio fibular fusion and fibular epiphysiodesis were performed in six children. An above knee caliper was used in 11 cases till graft hypertrophy and consolidation occurred. All patients are ambulant presently. Shortening was treated with a shoe raise. Shortening ranged from 1ā€“10cm. Fibula transfers showed hypertrophy with weight-bearing in all patients. Follow-up ranges from 10 months to 11 years. Conclusion: Onlay grafting was successful in small defects < 2cm. Larger defects up to 10cm can be treated by segmented bicortical iliac crest grafts over a Kirschner wire, providing some stability and increased surface area for incorporation. Bone transfers should be reserved for difficult cases with extensive scarring and as an alternative to ablation in selected cases

Object: To study the incidence, etiology and management of patients with neuralgia following Posterior Lumbar Interbody Fusion (PLIF). Design: A prospective study of 216 patients undergoing PLIF surgery from March 1996 to August 2003. Subjects: 16 of the 225 patients (7.1%) undergoing PLIF surgery developed new leg pain following surgery. Results In all patients, the distribution of the postoperative pain was different than the pre-operative pain. Nerve swelling with relative stenosis was the most common cause (9/16), followed by pedicular screw misplacement (2/16), nerve anomaly (2/16), loose posterior arch (2/16), and graft subsidence (1/16). Nine patients with nerve swelling complained of pain with no neurological findings. One responded to a root block and 6 to re-exploration and further decompression. The patients with misplaced screws woke up from surgery with pain and neurology. The pain responded to removal of screws in both. Two patients with loose posterior arches complained of leg pain while lying down only. These symptoms disappeared after excision of the arches. In 2 patients conjoint roots were found intra-operatively. Both developed post operative pain, which settled down in one. In one patient subsidence of an iliac graft caused pain a few months following surgery and did not respond to further decompression. Discussion: The overall incidence of post PLIF neuralgia was 7.1%, however by removing the whole facet, as performed in the last 122 patients, the incidence reduced from 9.7% (first 103 patients) to 4.9%. Based on our data we propose a classification system for Post PLIF Neuralgia, and a management plan. Using both, 11 of the 16 patients reported their leg pain disappeared, leaving only 5 patients with residual leg pain (< 3%). Key-stones in avoiding post PLIF neuralgia are generous decompression of the nerve roots, especially the lateral recess above the disc, total facetectomy and removal of graft pieces before closure

Purpose: The purpose of this work was to evaluate the mid-term clinical and radiological outcome of a homogeneous and continuous series of third-generation total ankle prostheses (resurfacing, cylindric, noncemented, triple-compartment). Material and methods: From March 1990 to June 1996, 26 patients aged 57 years (32ā€“73) were treated with a New Jersey LCS (n=5) or Buechel-Pappas (n=21) prosthesis. Most of the patients (n=21) had a posttraumatic ankle. Preoperatively, mean ankle motion was 17Ā°. The AOFAS score was used for the clinical assessment. The position of the prosthesis and its stability over time were assessed on the x-rays together with the insert, the bone-prosthesis interface, the bone structure and periprosthetic ossifications. Results: Intra and postoperative complications were malleolar fracture (n=5), haematoma (n=1), late wound healing (n=1), insert instability (n=2), and medial malleolar conflict (n=1) requiring reoperation with preservation of the implant. At mean follow-up of seven years, two patients had died and one was lost to follow-up. Three had an arthrodesis: failed fixation at two years, secondary talar mobilisation at seven years, and secondary infection at eight years. For the other twenty patients: the AOFAS score was poor for two patients (including one with patent wear), fair in two, and good in 16. Mean joint motion at last follow-up was 24Ā°. Radiographically, there were no significant changes in the position of the tibial and talar elements. Anchorage of the tibial element was fibrous in half of the patients and ossesous in the other half. A macro defect aspect was observed below the talar element in four patients. There were active periprosthetic ossifications in the majority of the patients. Discussion: The insufficient ancillary for this prosthesis and its old concept explain the frequency of malleolar fractures and the level of the functional outcomes. Improved prostheses should be used. The stability of the results observed in this series of patients followed up to 12 years is an argument favouring indicating prosthesis insertion as an alternative to arthrodesis, particularly since revision of an arthrodesis is not particularly difficult, even with an iliac graft. The radiographic evidence of periprosthetic ossifications or bone resorption, particularly under the implant, emphasises the need for prolonged surveillance

Purpose of the study: We conducted a prospective randomized study to compare use of macroporous biphasic calcium phosphate ceramic and bone autografts for filling medial open wedge osteotomies of the proximal tibia. Material and methods: This phase III pragmatic clinical trial was designed for direct patient benefit. Randomization was performed in the operating room after completing the osteotomy. Twenty-six men and 14 women, mean age 51 years (range 19ā€“75 years) were included. A biomaterial implant was used for 22 patients and an autograft for 18. Mean correction was 10 mm (range 6ā€“15). One patient was excluded from the analysis, no patient was lost to follow-up. All patients were reviewed at minimum two years follow-up. Results: At three months, knees were less painful with less subjective functional impact after filling with an autograft (pain VAS 3.1 versus 2.1 and function VAS 3.4 versus 2.5). These results were more balanced at six months (pain 1.6 versus 1.8 and function 1.8 versus 2.1) and remained stable at one and two years. The IKS knee scores were symmetrical at one at two years for both groups (IKS1 93 versus 86 and IKS2 90 versus 90). Bone healing was achieved within the usual delay. Axial alignment was stable at two years in both groups. There were eleven complications (28%), nine requiring revision: infected hematoma (n=1), intraoperative vascular injury without serious consequences (n=1), loss of correction (n=2), nonunion after filling with biomaterial (n=1), iliac abscess after filling with autograft (n=2) and painful calcification of the iliac region requiring resection (n=1). Discussion: We observed three factors which can favor mechanical failure after filling with a ceramic material: intraoperative rupture of the lateral hinge, obesity, and excessively early unprepared weight bearing. Conclusion: Although the difference did not reach significance, the risk of mechanical complications appears greater with macroporous en bloc ceramic filling. This material is less tolerant to comorbid conditions (obesity) and requires very precise technique as well as careful observance of postoperative care (no early weight bearing). Nevertheless, this method does have the advantage of avoiding painful sequelae and complications related to harvesting the iliac graft

Purpose: The purpose of this work was to demonstrate the usefulness of osteoplasty block for the treatment of primary hip osteochondritis. Material and methods: Eighteen children underwent surgery between 1992 and 2001. Mean age at diagnosis was eight years five months (5ā€“13 years). Preoperatively, mean motion limitation was āˆ’25 in abduction and internal rotation. Radiologically the series included: Catterall II Herring B with signs of high-risk head (n=2), III B (n=7), III C (n=1), IV B (n=1), IV C (n=2), and sequelar stage (n=2). Mean excentration was 6.5 mm and mean lateral overhang was 10.2 mm. Surgery was indicated because of an aggravation of the excentration with revascularisation. An iliac graft was encrusted in a limbic rail and covered with rectus femoris tendon left continuous to provide elastic support. A bermuda cast was used for 17 children with immediate weight bearing for eleven. Hospital stay was 36 hours for all children. Results: Sixteen children were reviewed at mean follow-up of three years two months. There were no complications. Clinically, there all patients were pain free, but three developed persistent limping. Ten had a moderate limitation of hip movement and three had severe joint stiffness. Radiologically, there was good head coverage in fifteen hips and joint congruency in fifteen, including eleven with concentric congruency. The intervention was considered useful for twelve children, and not useful for one who developed complete lysis of a poorly positioned graft. Usefulness was uncertain for three children, including the two with a sequelar-stage hip at diagnosis. Discussion: Femoral osteotomy to correct the valgus and pelvic osteotomy to reorient or widen the acetabulum are effective treatments but have many disadvantages (limited abduction movement, leg length discrepancy, re-intervention to remove material). The excellent integration of the plasty block at mid term is a sign of effective head-acetabulum adaptation. Clinical and radiological results have been similar to those obtained with other methods. Joint stiffness and postoperative limping are observed in very severe cases. Joint prognosis depends on the severity of the disease. Conclusion: This series of paediatric patients who have not yet reached bone maturity shows that the plasty block meets expectations for the treatment of early-stage primary hip osteochondritis. Treatment time is short and there are no complications

Purpose of the study: The proximal humerus is a common localization for solitary endchondroma. Levy (. Clin Orthop. 2004. , . 431. ) emphasized the frequency of associated muscle and tendon disease. Treatment is generally curettage-autograft filling. Use of calcium phosphate bone substitute has been validated (A. Uchida et al. J Bone Joint Surg (Br) 90, F. Gouin Rev Chir Orthop 95, R. Mirzayan J Bone Joint Surg (Am) 2001). This retrospective analysis was conducted to determine the signs and symptoms and report the results of surgical treatment obtained in a consecutive series of 15 patients with metaphyseal enchondroma treated in the same unit. Material and methods: This series included twelve women and three men, mean age 48.2 years (range 38ā€“73). All complained of pain. Two also had signs of calcification and six presented a cuff tendinopathy. Eight had had one or more joint injections. On average, the enchondromas measured 3.1 cm on the ap view and 3.6 cm on the lateral view. Magnetic resonance imaging (MRI) demonstrated the presence of a subacromial effusion in 13/16 shoulders, supraspinatus tendinopathy in six, calcifications in three, and acromioclavicular arthropathy in three. Curettage was followed by filling with biphased tricalcium phosphate (SBM, Lourdes) associated in nine shoulders with acromioplasty-bursectomy and in two with resection of a calcification. Results: There were no postoperative complications. Mean follow-up was six months. All patients recovered joint motion, seven were pain free, six complained of pain at exercise and two had episodic pain. There were no local signs of substitute intolerance. Follow-up was greater than one year in 12 patients and greater than two years in eight: seven shoulders were pain free, three presented pain at exercise, and two required analgesic drugs. Radiographically, the limit between the bone substitute and the cancellous bone was imprecise; the bone substitute could not be readily visualized in four shoulders, had faded out in three, and was visible in five. Discussion: The association of enchondroma and a rotator cuff pathology is common suggesting the tumor could affect disease expression. Imaging provides strong arguments favoring a benign disease. Use of bone substitute for filling is reliable and avoids the need for an iliac graft. Conclusion: A fortuitously discovered or painful enchondroma of the humerus should be treated by curettage-filling with bone substitute as soon as the nature of the tumor has been clearly identified and/or strong uptake on scintigraphy visualized. This is a supplementary operative argument suggesting an associated cuff pathology

Purpose: Posterior instability of the shoulder requires surgical treatment in involuntary forms and after failure of functional treatment. As for anterior instability, a bone block can be fashioned from an iliac graft or a pediculated graft harvested from the acromion. This retrospective analysis concerned the results obtained in eighteen consecutive cases. Material and methods: This series included ten men and eight women, mean age 26 years (15ā€“42) at time of surgery. Fourteen (77%) practised sports activities including four at competition level. Symptoms had persisted for four years on the average. For three patients (16%) posterior instability was expressed by recurrent luxation subluxation. For nine patients (50%) posterior subluxation was a common involuntary event. Six patients suffered from painful shoulders due to an unrecognised posterior instability accident. Diagnosis of posterior instability was establised by arthroscopy in seven patients (39%). For nine shoulders the intervention consisted in a screwed posterior iliac block associated with a tension procedure on the capsule (group 1). For the other nine shoulders the block was harvested from the acromion and pediculated on the deltoid (group 2). Results: Mean follow-up for all patients was eight years. The Duplay score was 75 points at last follow-up. Twelve patients (85% of the athletes) were able to resume their sports activities, half at the same level. Nine patients were pain free. Six patients (33%) had persistent apprehension but did not present true recurrence at physical examination. The Duplay score was 69.4 points in group 1 (follow-up 12 years) and 82.2 points in group 2 (follow-up three years). Thirteen patients (77%) did not have any sign of joint degeneration at last follow-up. One patient had advanced stage IV degeneration related to an intra-articular screw. All patients felt their shoulder had been improved by surgery and one third were disappointed with the results. Discussion: Use of a posterior block is an effective treatent for posterior instability giving results comparable with those obtained with anterior blocks in terms of shoulder stability, pain, recovery of motion, and subjective outcome. This procedure favours joint degeneration less than anterior stabilisation. Results obtained with pediculated acromial blocks are encouraging

Introduction: The occurrence of non-union following instrumented scoliosis correction may predispose to pseudarthrosis and subsequent implant failure. Although non-union is often multifactorial, it is widely accepted that bone graft of adequate quality and quantity is fundamental to achieve solid fusion. Conventionally, autologous rib graft or iliac crest harvest has been utilised for endoscopic anterior instrumented scoliosis surgery. However, these techniques increase the operative duration and cause donor site morbidity, both of which may lengthen hospital stay. Alternatives such as allograft bone and bone morphogenetic proteins have gained more widespread use and may improve fusion rates although this remains controversial. The aim of this study was to compare two-year postoperative fusion rates for a series of patients who underwent endoscopic anterior instrumentation for thoracic scoliosis utilising various bone graft types. Methods: 19 patients who had undergone endoscopic anterior instrumented scoliosis correction using identical instrumentation (4.5mm diameter titanium anterior rod and vertebral body screws, Eclipse, Medtronic) between May 2000 and August 2005 were identified from a surgical database of 132 consecutively treated individuals. All patients received bone graft to supplement thoracic fusion. Discectomy was performed at the levels to be instrumented and intervertebral spaces were packed with autologous rib heads (8 patients), iliac crest (1 patient), or mulched femoral head allograft (10 patients). The quality of thoracic fusion and implant integrity were evaluated two years following scoliosis correction using low-dose CT performed in accordance with local ethical approval. The intervertebral fusion was assessed using a modified Sucato method (1). Each level was graded using a 4-point scale based on calculated percentage of fusion across the disc space. 0 points indicated no fusion; 1 point, fusion < 25%; 2 points, fusion between 25 and 50%; 3 points, fusion between 50 and 75%; 4 points > 75% or complete fusion. The fusion was considered solid with a score of 3 points or more. Data was analysed with non-parametric tests using a significance level of 0.05. Results: Of the cohort, nine had evidence of implant failure with rod fracture. All implant failures occurred in the group who received either rib head or iliac crest graft. No rod fractures were identified in the femoral allograft group. The mean fusion grade in the autologous bone graft group was 1.91 whereas in the allograft group this was 3.30 (95% confidence intervals 1.38ā€“2.44 and 2.99ā€“3.61 respectively) with a statistically significant difference in fusion rates between these two groups (p=0.001). Discussion: This study demonstrated significantly better rates of thoracic fusion in endoscopic anterior instrumented scoliosis correction using mulched femoral allograft compared with autologous rib heads and iliac crest graft. This could be partly explained by the difficulty obtaining sufficient quantities of autologous graft. The lower fusion rate seen in the autologous graft group appears to predispose to rod fracture although the longer-term clinical consequence of implant failure in this group is not clear and warrants further study

1. This is a simple clinical study of the end-results of arthrodesis of the hip joint in patients followed up and re-examined five to twenty-five years after operation. 2. The study was stimulated by our astonishment at recent reports which suggested that arthrodesis of the hip caused serious operative mortality, a high rate of wound infection, and failure of sound fusion in one of every two cases; and that even when sound fusion was gained there was always pain in the back and usually stiffness of the knee. To say that we were astonished puts it mildly. 3. This review includes 120 patients aged from ten to seventy years, treated for osteoarthritis of the hip joint by intra-articular arthrodesis with the internal fixation of a nail, usually with an iliac graft, and with immobilisation in plaster for not less than four months. 4. Of these 120 patients there was sound fusion of the joint, proved radiographically, in 94 per cent; a mortality of nil; and recovery of free movement of the knee joint to the right angle or far beyond in 91ƂĀ·5 per cent. Almost half of the patients regained normal movement, the heel touching the buttock. Only in eight patients was there less than right-angled flexion. 5. There was no pain in the backĆ¢Ā€Ā”none whateverĆ¢Ā€Ā”in 64 per cent of the patients. In 36 per cent there was some pain or discomfort. One alone said that the low back pain was worse than before the operation. Many others said that pain in the back had been relieved by the operation. 6. It is emphasised that these results were gained only from sound fixation of the joint in the mid-position with neutral rotation, no more abduction than is needed to correct true shortening, and no more flexion of the joint than that with which the patient lies on the table. The limb was immobilised in plaster for at least four months after operation. The stiff knee was mobilised by the patient's own exercise without passive stretching, force or manipulation. 7. Two other groups of patients are considered. There are fourteen treated by fixation of the joint with nail alone, an operation that was never intended to arthrodese the joint and which has long since been abandoned. The other small group is that of patients with old unreduced traumatic dislocation of the hip, a procedure in which the risks of operation are so great and the number of successful results so small as to dissuade us from attempting operative reduction. 8. After successful arthrodesis of the hip joint patients can return to every household activity and every recreation including ski-ing, mountaineering, rock climbing, or whatever else they want


Aims

Our objective was to conduct a systematic review and meta-analysis, to establish whether differences arise in clinical outcomes between autologous and synthetic bone grafts in the operative management of tibial plateau fractures.

Methods

A structured search of MEDLINE, EMBASE, the online archives of Bone & Joint Publishing, and CENTRAL databases from inception until 28 July 2021 was performed. Randomized, controlled, clinical trials that compared autologous and synthetic bone grafts in tibial plateau fractures were included. Preclinical studies, clinical studies in paediatric patients, pathological fractures, fracture nonunion, or chondral defects were excluded. Outcome data were assessed using the Risk of Bias 2 (ROB2) framework and synthesized in random-effect meta-analysis. The Preferred Reported Items for Systematic Review and Meta-Analyses guidance was followed throughout.

We report the results of limb salvage for non-metastatic osteosarcoma of the distal tibia using resection arthrodesis, autogenous fibular graft and fixation by an Ilizarov external fixator. In six patients with primary osteosarcoma of the distal tibia who refused amputation, treatment with wide en bloc resection and tibiotalar arthrodesis was undertaken. The defect was reconstructed using non-vascularised free autogenous fibular strut graft in three patients and a vascularised pedicular fibular graft in three, all supplemented with iliac cancellous graft at the graft-host junction. An Ilizarov external fixator was used for stabilisation of the reconstruction. In five patients sound fusion occurred at a mean of 13.2 months (8 to 20) with no evidence of local recurrence or deep infection at final follow-up. The mean post-operative functional score was 70% (63% to 73%) according to the Musculoskeletal Tumour Society scoring system. All five patients showed graft hypertrophy. Union of the graft was faster in cases reconstructed by vascularised fibular grafts. One patient who had a poor response to pre-operative chemotherapy developed local tumour recurrence at one year post-operatively and required subsequent amputation

In developmental dysplasia of the hip, a deficient acetabulum may be augmented by placing local autogenous iliac osseous graft, or the ilium itself, over the head of the femur with the expectation that the added bone will function as a bearing surface. We analysed this bone obtained en bloc during subsequent surgery which was performed for degenerative osteoarthritis in three patients at 6, 25 and 30 years after the initial augmentation procedure. In each patient, the augmentation comprised of red cancellous bone covered on its articulating surface by a distinct layer of white tissue. Microscopy of this tissue showed parallel rows of spindle-shaped cells lying between linearly arranged collagen bundles typical of joint capsule. Biochemical analysis showed type I collagen, the principal collagen of joint capsule and bone, with no significant quantity of type II collagen, the principal collagen of cartilage. While the added bone produced by acetabular augmentation was durable, histological and biochemical analyses suggested that it had not undergone cartilage metaplasia. The augmented acetabulum articulates with the head of the femur by means of an interposed hip joint capsule

Gold standard for the management of non-union is open surgical debridement, stabilisation, and autologous bone grafting. LIPUS is becoming more popular, yet the evidence is still inconclusive. LIPUS involves the use of ultrasound at the fracture site with little risk to the patient. The purpose of this study was to assess effectiveness and cost benefit of LIPUS in the management of non-unions post sustaining an open fracture. We retrospectively reviewed 29 patients with open fractures with established non-union undergoing LIPUS since 2010 (4 females, mean age 48) range 3ā€“27 months, mean 9 months, either post injury or last intervention. All were tertiary referrals, sustaining injuries to the following areas; Tibial 21, Femur 6, Humerus 2, Radius 1. Definitive fixation being; 9 TSF's, 11 IMN's, 9 plates. (undergoing a mean 2.4 procedures). Aside from sustaining an open fracture, 7 had risk factors for non-unions 5 smokers, 2 NSAID's. Failure of treatment was based on undertaking bone grafting. In 28 patients (1 lost to follow up) union was achieved in 71% (mean 157 days). All were screened for infection, 4 had organisms on enrichment culture. 8 (5 Gustillo Anderson Grade 3A/B) injuries did not show evidence of callus formation, LIPUS was discontinued and grafting performed. Open fractures were graded as; 7 Grade 1, 4 Grade 2, 8 Grade 3A, 10 Grade 3B being received. Of these; 20 underwent primary closure, 6 free flaps and 3 SSG. The cost of LIPUS is approx Ā£2500, compared bone grafting using autologous iliac crest graft with no medical comorbidities of Ā£3715. This case series further supports union rates after LIPUS. Cost and morbidity benefit of utilising LIPUS over opting for bone grafting initially is Ā£1215 per patient. Whilst autologous bone grafting is currently the gold standard, it is not without morbidity. We achieved union rates of 71% despite a number of patients having recognised risk factors, showing that LIPUS is a useful resource in the management of non-union

Acute osteomyelitis of the radius or ulna in children is rare and may be associated with complications including pathological fracture, growth disturbance and cosmetic problems. Purpose:. To highlight the outcome of acute pyogenic osteomyelitis of the forearm bones in children. Methods:. Eleven children were treated for osteomyelitis of the radius (6) and ulna (5) over 15 years. Staphylococcus aureus was cultured following initial incision and drainage. Two had signs of compartment syndrome. Late complications included gap defects of 2ā€“6 cm (radius 1 and ulna 2). Larger defects with physeal involvement were seen in the distal ulna (4) proximal radius (1) and whole radius (1). The late clinical features included pseudarthrosis (9), distal radioulnar instability (3), radial head dislocation (3) and ā€œradial clubhandā€ type deformity (1). Treatment:. Gap defects <2 cm were filled with autogenous grafts (3). Segmented iliac crest grafts threaded over a K wire were used in 1 patient with an 8 cm gap defect. Radio-ulnar synostosis was performed in 4 cases. The carpus was centralized onto the ulna in 1 child. Results:. Reconstructive grafts healed by 6ā€“12 weeks. Residual elbow contracture <30Ā° occurred in 3 children. Ten children had improved grip strength and stability of the wrist and elbow and forearm length was decreased by 2ā€“5 cm. The child with a radial clubhand deformity had severe shortening and stiffness of the hand. Conclusion:. Osteomyelitis of the forearm bones can be missed and present late. Complications include disproportionate growth, proximal or distal radio-ulnar instability and radial clubhand type deformity. Treatment is challenging requiring reconstruction of gap defects. Radio-ulnar synostosis is a useful salvage procedure to improve function and cosmesis

Scaphoid injuries remain a challenge in both diagnosis and subsequent management. Untreated scaphoid non-union leads to inevitable osteoarthritis. Non-unions are picked up via an established management pathway in out trust and treated surgically with a standardised technique of non-vascularised bone grafting and retrograde screw fixation. Patients that underwent primary screw fixation and grafting for scaphoid non-union between 2004-2008 were reviewed. These patients were followed up clinically and radiologically. They were assessed for time to union, complications, DASH score, patient satisfaction and return to work. In addition comparison was made between the use of Herbert screw and Twinfix screw fixation and between tricortical iliac crest graft and corticocancellous distal radial bone graft. Forty-two consecutive patients underwent surgery of which 35 (83%) were available for follow up. Mean follow up was 39 months (range 13-72). 31 (89%) patients had gone on to radiological union. Of those united the mean DASH score was 11. Mean time to union was 4.4 months (range 2-15 95% CI 3.3 ā€“ 5.5). 95% of patients were satisfied with their operation and would recommend it to a friend in the same situation. No significant difference in outcomes were identified between Herbert screw and Twinfix screw fixation or between iliac crest or distal radial bone grafting techniques. This study has shown encouraging results for an established protocol for management of scaphoid non-union. Outcomes are as a good as any published comparable series in the literature and provide further support to the use of this technique

To assess radiological fusion rates in posterolateral fusions using SiS-CaP. Retrospective, radiological follow-up study. Single surgeon series of 76 consecutive patients were evaluated, in a regional spinal unit. All patients had clinical and radiological (MRI) spinal canal stenosis secondary to degenerative spondylosis or spondylolisthesis. Surgery consisted of instrumentation, decompression and meticulous preparation of the posterolateral graft bed by removal of all soft tissues posterior to the inter-transverse membrane and decortication of transverse processes (TPs). SiS-CaP putty was injected into this gutter and moulded around the instrumentation. Good quality, well prepared bone chips from the posterior decompression were seeded into the putty. Patient radiographs were reviewed at 3-6 months, 1 year and 2 years. Radiographs were assessed using a protocol to examine granularity, bone formation and evidence of pseudarthrosis, based upon previously reported literature . 1. and our personal experience. Of the 76 patients, 26 were excluded. M:F was 21:29. Mean age was 58yrs. Average number of motion segments fused per case was 2.2. There was one pseudarthrosis with metalwork fracture, and thus a total fusion rate of 98%. In addition, one patient had scanty bridging of TPs, and one patient had lucency around the S1 screws. SiS-CaP, as a bone graft substitute in posterolateral instrumented fusions, gives comparable results to published fusion rates using autologous iliac crest grafting and/or Bone Morphogenic Protein . 2. Moreover, it avoids the associated morbidity of iliac bone harvest

Introduction. Significant donor site morbidity (3ā€“61%) has been associated with tri-cortical iliac crest bone graft harvesting and reconstruction of the defect has been shown to reduce it. Chitra-HABG (Chitra-Hydroxyapatite-Bio-active glass ceramic composite) is an indigenously developed ceramic which has been evaluated as a bone graft substitute. Aim. To prospectively validate the hypothesis that iliac crest donor site morbidity is a structural issue and reconstructing the crest reduces its incidence. The study also evaluates the efficacy of Chitra - HABG as a material for reconstructing the crest. Methods. 26 cases of iliac crest reconstruction with Chitra-HABG blocks were prospectively evaluated. Outcome measures were donor site morbidity as assessed clinically and radiological assessment for ceramic incorporation, dissolution, fragmentation and migration. Results. At the end of one year from surgery, 25 patients (96.15%) had no donor site pain. Radiological evaluation showed that ceramic incorporation was complete in 21 cases, partial in three and absent in two. Partial dissolution of ceramic was noticed in three patients and migration in one. Conclusion. The present study validates our hypothesis that the donor site morbidity following tri-cortical iliac crest graft harvesting is probably a structural issue and it can be reduced by reconstruction of the defect. It also highlights the fact that the Chitra-HABG block is an excellent material for reconstruction of the iliac crest defect, as it gets incorporated into the surrounding bone without adverse effects. Significance. Study describes a simple and effective method to decrease iliac crest donor site morbidity. Level of evidence: IV. Study Design. Prospective study. Summary. Introduction and Objectives: The aim of this study was to assess the results seen in patients with fracture of the tibial plateau treated by closed reduction and osteosynthesis under arthroscopic control. It is also the aim of this study to vindicate the use of the arthroscopic technique. Materials and Methods: We carried out a retrospective study of 17 patients with external tibial plateau fracture Shatzker type I, II and III, treated by reduction and percutaneous osteosynthesis under arthroscopic control. We performed a metaphysis fill with iliac crest graft material. We determined the incidence of associated intraarticular lesions. Mean follow-up was 27 months. We assessed range of mobility, pain (VAS), muscular force with an isokinetic test and joint status by means of the Rasmussen scale. Results: Mean age was 38 years with a mean time to intervention of 8 days. We found associated intraarticular lesions in 8 cases, 7 meniscal ruptures and 1 ACL rupture. The mean range of mobility was 125Ā°. The mean loss of force was 15% in comparison with the contralateral leg determined by isokinetic studies. Results were satisfactory in 85% of cases according to the Rasmussen scale. Discussion and Conclusions: The treatment of fractures of the external tibial plateau, Schatzker type I ā€“ II ā€“ III by closed reduction under arthroscopic control makes it possible to diagnose and treat associated intraarticular lesions and obtain good to excellent functional results in most cases

Aims

To report the surgical outcome of patients with severe Scheuermannā€™s kyphosis treated using a consistent technique and perioperative management.

Methods

We reviewed 88 consecutive patients with a severe Scheuermann's kyphosis who had undergone posterior spinal fusion with closing wedge osteotomies and hybrid instrumentation. There were 55 males and 33 females with a mean age of 15.9 years (12.0 to 24.7) at the time of surgery. We recorded their demographics, spinopelvic parameters, surgical correction, and perioperative data, and assessed the impact of surgical complications on outcome using the Scoliosis Research Society (SRS)-22 questionnaire.

Introduction and purpose: The purpose of this study was to assess the functional results of ankle arthrodesis as a treatment of posttraumatic sequelae. Materials and methods: We carried out a retrospective study from the 1st of January 2000 to the 31st of December of 2005 with a total of 55 patients that underwent posttraumatic ankle arthrodesis, with a mean follow-up of 5 years (range: 1ā€“7 years). The mean age of the patients was 40 years (range: 18ā€“61). There were 52 men and 3 women, the right side (40) predominated over the left (15). Seventy-eight percent of trauma sustained was high-energy with a predominance of fracture of the tibial pilon (type C3 fractures on the AO classification). There were 35% open fractures. The predominant type of surgical technique performed used cannulated screws without any iliac crest grafts. The mean time from surgery to discharge was 9 months (range 4ā€“19). Results: Repeat arthrodesis was necessary in 16% and an infrapatellar amputation was necessary in 1 case. Only 1 patient returned to their workplace without limitations, the rest were discharged to a medical board for sequelae. The use of crutches/insoles and subjective assessment of pain were related to a lateral tibiotalar angle greater than 90Ā° and varus deformity. Conclusions: Tibiotalar arthrodesis is a useful rescue technique in patients with severe posttraumatic ankle sequelae. It eliminates pain and increases stability but is the cause of a significant loss of function of the lower limb

Introduction: We present our experience in the treatment of the aseptic necrosis of the femoral head with a vascularized iliac crest graft by means of a retrospective analysis of 25 hips (22 patients) subjected to surgery in our hospital between June 1993 and September 2002. Materials and methods: There was a predominance of male patients (20 cases). In most patients the necrosis appeared in the left side. The most common predisposing factor was alcohol abuse (8 cases). All patients were subjected to a clinical (Harris score) and a radiological evaluation, and the evolution of their necrosis was assessed using the Ficat-Arlet system (as modified by ARCO) with a mean follow-up of 4 years. Results: The average functional evaluation was of 87 points, with 76% of results in the excellent and good categories and 24% fair and poor. 40% of the hips underwent some kind of radiological deterioration (mean: 5.2 years). On the basis of the radiological evaluation criteria, 66% of patients evolved satisfactorily. Conclusions: Our results are in line with those published in the literature. 60% of patients operated on showed no evidence of a radiological progression of the necrosis of the femoral head

This study reports the 5-year clinical and radiological outcomes of a simple arthroscopic-assisted technique for Schatzker type II and III tibial plateau fractures, without bone grafting. Forty six patients (46% males, 54% females, average age 48 years, SD 13.6 years), with tibial plateau fractures Schatzker type II (41%) and III (59%), underwent an arthroscopic-assisted technique conceived to use a compacted cancellous bone graft, taken from the medial metaphyseal side of the tibia, and a percutaneous fixation. The patients were prospectively followed-up at one, three and five years from surgery. Independent assessments were carried out using Knee Society Score, HSS score and Rasmussenā€™s clinical and radiological scores. At 5-year follow-up patients underwent a weight-bearing radiograph of both limbs. At last follow-up evaluation Knee Score (average 93.2, SD 7.7) was excellent in 37 patients (80%), good in six (13%), fair in three (7%). Function Score (average 94.8, SD 8.51) was excellent in 38 patients (83%), good in five (11%), fair in three (6%). HSS score (average 93.4, SD 8.23) was excellent in 41 patients (89%), good in five (11%). The average Rasmussen clinical score was 28.2 (SD 1.4). The radiological Rasmussen score was excellent in five patients (11%), good in 39 (85%) and fair in two (4%). In the weight-bearing radiographs a valgus deviation was present in four patients (8.7%). Arthroscopic-assisted technique for lateral tibial plateau fractures without bone grafting has outcomes encouraging and comparable to the results of other techniques that use either iliac crest graft or bone substitutes

Background. The aim of the study was to assess the outcome of internal fixation using two ordinary plates with autogenous bone grafting in the management of non-union of femoral shaft fractures over a period of eleven years. Method. A retrospective study of double plating and autogenous bone grafting for un-united femoral shaft fractures performed by the two Senior Authors between 1996 and 2007. During this period, twenty-one cases were carried out. Details of preoperative, intra-operative, and postoperative findings were assessed. Postoperative follow up was recorded over a period of minimum two years. The procedure was performed using two ordinary plates; a broad lateral and narrow anterior plates. Autogenous cancellous grafts from iliac crest were used in all of the cases, and in some, half fibulas were inserted through the medullary canal of the femur. Results. A total of twenty-one patients were included in the study. All of them received previous treatment. The primary treatment was by intramedullary nailing in thirteen patients, uni-plating in four, external fixator in three, and traction in one. The average age was 32 (range from 16 to 52). The average interval between the initial fracture and the treatment of the non-unions was 13 months (range from 6 to 28 months). All wounds, both at fracture site and the graft donor site, healed primarily except one. That was at an iliac crest graft donor site, where superficial infection occurred, and was treated with antibiotics and healed 6 weeks later. All patients achieved union on an average of 16 weeks (range from 12 to 28 weeks). There has been no failure in implants. The average lower limb shortening was 0.75 cm (range from 0.5 to 1.0 cm). No one requested shoe raise. All the patients regained the preoperative range of motion at the hip and knee joints except four, who had limited flexion to 100 degrees. Conclusion. We conclude that double plating with autogenous bone grafting is not only a convenient method to treat non-union of femoral shaft fractures as it requires minimal instrumentation and equipment, but it should also be considered as a safe and accepted treatment method as postoperatively patients can be discharged early on crutches, with a high success rate. It is less expensive, an important consideration in developing countries

Aims: Failure after arthroplasties of Ć¾rst metatarso-phalangeal joint (MPJ) is relatively common because the instability and the insufĆ¾ciency of the great toe; the Patients usually complaint for transfer metatarsalgia and secondary deformities of the lesser toes. The goal of the study is to evaluate a series of arthrodesis of Ć¾rst MPJ performed for failed Keller or Hueter procedures; in all cases bone grafts were used in order to restore the normal lenght of the Ć¾rst ray. Methods: Between 1996 and 2002 16 Patients were treated with this technique; there were 14 cases of failed Keller arthroplasty and 2 cases of failed Hueter procedure. In all cases tibial or iliac cortico-cancellous grafts were used. Stabilization was performed by 1.5 mm. K-wirws. In 10 cases additional procedures about lesser rays were used. Results: Evaluation of the results was made using AOFAS forefoot score with a minimum follow-up of 7 months (average 3.3 yrs). Fusion was achieved in all cases but one. Average time of fusion was 9 weeks. All Patients were satisfyed with the results in term of pain relief, function and cosmesis. Conclusions: Interposition graft arthrodesis is an effective procedure for failed arthroplasties of Ć¾rst MPJ


Study Design. Single surgeon prospective cohort with radiological follow-up. Background. Anjarwalla et al. have shown that the addition of posterior pedicle supplementation without posterolateral fusion during an ALIF procedure significantly increases the rate of interbody fusion when using a carbon fibre / PEEK cage packed with autogenous iliac crest graft. Stand alone ALIF cages which utilise screws passing through the interbody cage and into the vertebral bodies were designed to obviate the need for a posterior procedure by increasing the anterior construct stability and fusion rate. Objective. To assess the effect of posterior pedicle screw supplementation without posterolateral fusion on the fusion rate of ALIF when using anterior cage and screw constructs. Method. Between 2003 and 2008 91 patients underwent ALIF procedure under a single surgeon for low back pain associated with degenerative disc disease or lytic spondylolisthesis. Routine CT was performed at around 1 to 2 years follow up to ensure union prior to discharge. It was noted that there was a significant number of locked pseudathroses, especially in 2 level cases, and in 2006 the surgeon started to perform simultaneous supplementary posterior pedicle screw stabilisation without posterolateral fusion in the majority of cases. Results. Of the 91 patients, 77 (85%) had CT follow up at mean 21 months. 45 patients had anterior surgery alone (26 single level and 19 two level) and 32 had front back surgery (15 single level and 17 two level). Pseudarthrosis was seen in 16 patients (36%) of the anterior alone group and 2 (6%) of the front back group. In the anterior alone group, pseudarthosis occurred in 6 / 26 (23%) patients with single level surgery and 10 /19 (53%) of those with two level surgery. In this last group, the pseudarthosis occurred in the upper fusion level in five patients and at both fusion levels in other five. Conclusion. Posterior pedicle screw supplementation without posterolateral fusion improves the fusion rate of ALIF when using anterior cage and screw constructs. We would recommend supplementary posterior fixation especially in cases where more than one level is being operated

Aim: To evaluate the outcome of late anterior decompression in patients with dorsal and lumbar spinal injuries with neurological deficit. Background: Anterior decompression and bone graft stabilisation of the spinal injuries allows direct decompression of the spinal canal and provides favourable environment for neurological and functional recovery. Proponents of both early and delayed decompression have shown favourable results. However, what is unclear is the timing of the surgery. Methods: A prospective study of 12 patients with spinal injuries, who had anterior decompression a minimum of 4 weeks after the injury (mean 7.5 weeks). 5 had incomplete and 7 had complete neurological deficit at presentation. The indication for the operation was persistent neurological deficit with retropulsed fragment of bone causing canal compromise. Anterior stablisation after decompression was by means of a tri-cortical iliac crest graft or a rib graft. Results: 8 males, 4 females with average age 26.8 years. 7 lumbar and 5 dorsal spine injuries. Average follow-up of 5.5 years with minimum of 5 years. Post-operative improvement was seen only in patients who sustained injury at the lumbar level, with 6 of the 7 patients regaining normal bladder and bowel function after decompression. Immediate post-operative improvements obtained in the Kyphotic angle were not maintained probably due to the settling of the graft, so posterior or anterior stabilisation may be needed in addition to anterior bone grafting to prevent worsening of the kyphotic angle. Conclusion: Delayed anterior decompression of the lumbar spine in patients who had spinal fractures, is an effective procedure, which may help neurological recovery, especially of the bowel and bladder function

In oncological resections there is a higher risk of infection around the foot and ankle. An infection here can be difficult to treat and easily lead to an amputation due to the limited amount of soft tissue coverage of the region. In three patients an infection developed after resection of a bone tumour in the foot and ankle. In the first case, female 34 years, an epitheloid hemangioepithelioma was excised from the anterior part of the calcaneus, cuboid and lateral os cuneiform. An iliac crest graft was initially used to fill the defect, but got infected. The antibiotic loaded bone cement spacer cured the infection and filled the dead space but was painful. A free vascularised fibula with skin-flap was used successfully to fill the defect and take away the pain. At three-year follow-up there is no pain and full weight bearing, with a nice hypertrophy of the graft. In the second case, a 14-year old girl, there was an Aneu-rismal Bone Cyst (ABC) of the distal tibia with a deep infection after ethibloc injection. The vacuum assisted closure cleaned the wound but a defect resulted. It was successfully filled with an ipsilateral free vascularised fibula with skin-flap. Follow-up shows full function and nice hypertrophy at 24 months. In the third case, male 65 years, a chondrosarcoma grade one (after biopsy) in the cuboid was curetted out. It proved grade two in the definitive histology and furthermore it got infected. The cuboid was excised and a cement spacer was placed. The soft tissues were insufficient to close it properly. A free vascularised fibula with skin-flap was used. The vascularity of the graft was insufficient and the skin-flap did not survive. A vacuum assisted closure was done. He can bear weight and has no pain. The fibula graft is shows some hypertrophy and a fistula persists for 18 months now. We conclude that vascularised free fibula with skinflap can successfully prevent amputation in case of infection in oncological resection of foot and ankle. The fibula reconstructs the bone defect and the skin-flap the soft tissue defect

Introduction and purpose: The VIRA system allows reconstruction of the fractured bone and its arthrodesis with the talar bone eliminating all movement of the subtalar joint by means of a minimally invasive approach. The aim is to restore the shape of the foot and prevent joint degenerative changes in the long term without the complications and sequelae of open surgery. Material and methods: Between November 2004 and June 2006, 30 intraarticular calcaneal fractures in 25 patients were treated surgically by means of the Vira System. Using Sandersā€™ classification 6 cases were classified as type II, 13 as type III and 11 as type IV. Five patients had open fractures. There was a mean period of 8.7 days between the accident and surgery. Seven cases had associated lesions and fractures in other locations. Only 2 cases required iliac crest grafts in the operated area. Results: All patients except 10 have returned to their usual occupations. Eight cases evolved favorably but have had a short follow-up. Two cases experienced a delayed healing of the subtalar arthrodesis and had to be reoperated; they required an autologous graft. The mean period of temporary disability in patients discharged from hospital was 163.7 days. Clinical assessment using the AOFAS scale reached a mean value of 76.6 points. No surgical or post-surgical complications were seen in the group studied. Conclusions: The VIRA System seems capable of achieving its purpose in the first series of patients operated. It allows a quick recovery both postoperatively and in the subsequent evolution without any associated complications. It accelerates the patientsā€™ return to work by decreasing convalescence and minimizing sequelae

Introduction and purpose: The ulnar carpal impaction syndrome (UCIS) is a common cause of pain in the ulnar aspect of the wrist. It has numerous causes, although most cases are due to rupture of the triangular fibrocartilaginous complex (TFC), either traumatic or degenerative. Materials and methods: We carried out a prospective study of the results of ulnar shortening osteotomy in 41 wrists. The osteotomy (transverse) was performed in the middle third of the ulnar and the shortening ranged from 3 to 10 mm. For stabilisation we used a 3.5-mm AO LC-DCP and LCP plate. Minimum follow-up was 6 months and maxim was 8 years. Results: The mean age was 37; there were 28 women and 12 men. The ulnar variance ranged from 5 mm positive to 2 mm negative. All the wrists had Tolat type I or II distal radioulnar morphology. Pain and pain frequency were reduced to levels below surgical indication in 89% of cases within 2 and 4 months after the operation. All the ulnae consolidated satisfactorily between 12 and 14 weeks, except for one case of pseudoarthrosis, which was treated with an iliac crest graft and further bone synthesis. Conclusions: The results obtained in this series reveal adequate progression of wrist pain and function in patients treated for UCIS by ulnar shortening. There were very few complications and revisions

Aims: To determine whether the use of Biphasic Calcium Phosphate (BCP) plus local autogenous graft modifies our clinical results and fusion rate, in comparison with iliac crest graft alone. Methods: Prospective and nonrandomized study. 40 patients were operated on with pedicle instrumentation and posterolateral fusion. Degenerative lumbar disease was the etiology. 25 patients had one level fusion and 15 two or more. Minimum follow-up: 1 year. 15 patients were smokers (17.4 cig/day). The Oswestry disability index at 0, 3, 6 and 12 months was filled in to see the evolution and final clinical result. To evaluate the fusion, X-ray criteria were used. Results: Oswestry test changed from 63,8 to 16,4 at the end of the study. 3 patients did not achieve fusion (7,5%), none of them smokers. Up to date we have been working with the same idea but trying to increase the fusion rate by adding bone marrow to BCP and grafts. The initial results are very encouraging. Conclusions: We have no difference in our non-union rate between previous historic register and the new technique. Similar clinical results were obtained but the proportion of excellent/good has improved with iliac crest preservation

Purpose of the study: Complex fracture-dislocation of the elbow, and subsequent surgical treatment, is often the source of a stiff joint. The purpose of this study was to assess the efficacy of a therapeutic protocol combining systematic insertion of a dynamic external fixator allowing early mobilization of the elbow with restitution of the radial height and the coronoid process. Material and methods: This consecutive series of ten patient, six men and four women, mean age 49 years, age range 27ā€“67 years, underwent surgery from 2002 to 2004. Three patients presented a posterior Monteggia fracture (two type IIA, one type IId associated with comminutive fracture of the trochlea), four patients presented a dislocation associated with a Masson 4 fracture of the radial head and two presented inveterated dislocations diagnosed three weeks after the traumatic event. In all, seven patients presented a fracture of the radial head and six a fracture of the coronoid process. For all patients, the operation consisted in stabilization with a dynamic external fixator of the elbow associated or not with restoration of the radial height with a radial head prosthesis (n=4) and reconstitution of the coronoid process (n=6). The lateral ligaments had to be reinserted in four elbows. The comminutive fracture of the rochlea was treated with an iliac crest graft. Results: One patient died early. One patient presented pin tract infection and four developed heterotopic ossifications. At mean follow-up of twelve months, outcome was excellent in our patients, good in four, fair in one (Mayo clinic classification). All patients had a stable elbow. The mean range of motion was 89Ā° flexion-extension and 145Ā° pronationsupination. Conclusion: In this series, systematic use of external fixation for complex fracture-dislocation of the elbow joint yielded satisfactory results when the element stabilizing the joint were appropriately restored and when rehabilitation was undertaken early

Aims: a) To review donor site morbidity and record problems coming out during bone harvesting, b) to evaluate bone graft adequacy and effectiveness. Methods: Senior author operated 127 patients using cancellous bone autografts (from May 1988 to September 2001). Bone grafts were obtained from iliac crest (83 patients), proximal tibial metaphysis (39 patients) and other donor sites (5 patients). In all cases intraopera-tive problems and early postoperative problems were recorded. Late postoperative problems were evaluated either subjectively (questionnaire) or clinically for 109 patients. Minimum length of follow up was 12 months. Results: In all the cases, the goals of graft harvesting were accomplished. Generally, grafts were adequate in quantity and quality (apart from 2 cases in which grafts were taken from proximal tibial metaphysis). Blood loss from the proximal tibia was less than iliac bone. Iliac pain was more severe than tibial, resulting to delayed mobilization and pain could persist for a long period of time. Conclusions: Proximal tibial metaphysis, as a donor site, is associated with far less morbidity than iliac and it should be chosen in patients with lower-extremity fractures or nonunions. Harvesting from tibia is adequate and easier comparing to iliac. Tibial bone grafts were as effective as iliac, despite that iliac bone graft is widely used and generally referred as a better quality autologus bone graft

Aim: To improve our technique of subtalar fusion for the correction of flexible abducto-valgus foot deformities during single event multilevel surgery in children with cerebral palsy, specifically, to decrease operating time, avoid donor site morbidity, improve fusion rates and prevent loss of correction. Methods and Results: The modifications are:. Preparation of the sinus tarsi using hemispherical reamers. Grafting the sinus with a press fit, cortico-cancellous iliac crest allograft, cut with a circular ā€œcookie cutterā€. Placing an 8mm titanium cannulated screw parallel to the talo-navicular and calcaneo-cuboid joints. All patients had a preoperative instrumented gait analysis, video recordings of foot position, AP and lateral weight bearing radiographs. Patients were encouraged to weight bear as tolerated in a cast at three weeks. Six weeks postoperatively the casts were replaced with ankle-foot orthoses. Gait laboratory reviews were conducted at 3, 6, 9, 12 and 24 months postoperatively with radiological evaluation of the subtalar fusion at 6, 12 and 24 months postoperatively. Between January 2001 and December 2004, 58 children, mean age 9 years and 4 months underwent bilateral subtalar fusions with the modified technique during multilevel surgery. Fifty-eight patients were reviewed 12 months postoperatively and 54 at 24 months postoperatively. Deformity correction was uniformly satisfactory. All patients had complete graft incorporation on plane radiographs. No patient required a revision procedure. Six patients subsequently had first metatarso-phalangeal joint fusions for symptomatic hallux valgus. Conclusion: The reliability of extra-articular fusion of the subtalar joint improved dramatically using screw fixation and iliac crest graft as described by Dennyson and Fulford. This can be further improved by the use of hemispherical reamers to the sinus tarsi and cannulated screws for fixation of the talus to the os calcis. Allograft is a reliable substitute for autograft and is acceptable to most patients and families

Aims: In reconstruction of brachydactyly, either post-traumatic or congenital, we present our method of skeletal lengthening by distraction. Methods: We treated 32 short digits in 15 patients. Posttraumatic: 10patients and Congenital shortening: 5patients (Average age 5years (3.4 ā€“ 7.7 years) in congenital group). For traumatic amputation duration between injury and surgery: average 14weeks (10ā€“19.5weeks). Concerned phalanx exposed through dorsal approach. Transfixing k-wires passed perpendicular to phalangeal shaft above and below proposed osteotomy and connected on either side to a distractor. Latency period: 5days. Distraction rate: 0.8 mm/day. Serial x-rays at 3weekly intervals. In 6patients with deficient new bone, bone grafting was done using iliac crest graft. Fixator left in situ till radiological evidence of bony healing than wires removed under local anaesthesia and patients were started on vigorous physiotherapy for gaining range of movements. 8patients required deepening of the web space in addition to above-mentioned procedures. This procedure was required in cases having more than one short digit. Results: Average length gained: 18mm. Average duration of distraction: 42.26days. Fixator in situ: average 92days (80ā€“136days). Initial stiffness of digits was seen in 50% of cases, which eventually responded to physiotherapy, & all patients had an excellent range of motion. Conclusions: This method provides patients with good function and cosmetic hand. Any surgeon not familiar with microvascular surgery can use this technique

Purpose: Harvesting of iliac crest graft for spinal fusions is associated with a number of patients reporting residual pain at the harvest site. Various interventions have been proposed to minimize the pain associated with this. The purpose of this study was to evaluate the efficacy of intra-operative infiltration of morphine into the iliac crest harvest site for reduction of postoperative pain. Method: A double-blinded, placebo controlled study was performed comparing intraoperative infiltration of 5mg of morphine (treatment) vs. saline (placebo) into the iliac crest harvest site for patients undergoing elective spinal surgery. Patients with myelopathy, excessive preoperative opiod use (60mg equivalent morphine/day or more), or multilevel (greater than 3 levels) spinal surgery were excluded. Postoperative administration of morphine (recovery room and PCA) was standardized. Numerical pain scores specific for the iliac crest site were determined in the immediate postoperative period and at 3, 6 and 12 months. Ad hoc power analysis demonstrated that 36 patients would be able to detect a clinically significant difference of 2.4 points on a 10 point numerical pain scale (alpha = 0.05, power = 80%). Results: 37 patients were randomized and evaluated with a minimum of one year follow-up. The groups were similar in baseline age, gender and comorbidity. There was no significant difference between groups in total use of postoperative morphine during the first 24 hours (treatment: 320mg +/āˆ’ 268mg, placebo: 293mg +/āˆ’ 182mg, p = 0.73). There was no significant difference in iliac crest activity pain scores at 1 year (treatment: 1.7 +/āˆ’ 3.0, placebo: 1.1 +/āˆ’ 2.4, p = 0.48) or at any of the earlier time points. The proportion of patients with moderate to severe iliac crest pain at one year follow-up was the same between both groups. Conclusion: This study has demonstrated that there are no additional benefits for the use of intra-operative infiltration of morphine into the iliac crest harvest site during spinal fusions

Hip pain in cerebral palsy is regarded to be underreported. Management of these patients at home is difficult as the patients mature. In the ā€˜non walkerā€™ category, the aims of surgery are to relieve pain and to allow sitting and transfer. Neuromuscular hips may have variable acetabular deficiencies ie) anterior /posterior / lateral. Many forms of surgical management, of varying complexity, have been described to address these problems. To describe a new technique with multidirectional coverage that achieves pain free hips, 15 patients were reviewed over a 4 year period. Inclusion criteria :- 1 Subluxated / dislocated hips with hip pain. 2 Patients who have failed conservative management. 3. Those not suitable for redirectional osteotomies. A standardised technique was performed by one surgeon, at one institution. In summary, the technique involves initially a standard derotation varus osteotomy. Via an anterior approach, a lateral iliac unicortical graft and strips of cancellous graft are harvested. The cancellous graft is laid on top of the intact capsule, in the areas of deficiency. The cancellous graft is held by the unicortical graft with a single screw. 15 patients were reviewed. Patients were categorised as ā€˜walkersā€™ (3) and ā€˜non- walkersā€™ (12.) The mean age was 13.2 years. All patients were pain free after recovery. This was defined as not requiring analgesia and parental satisfaction. The radiological appearances showed that all the shelves had incorporated, with satisfactory cover of the femoral head. This technique addresses multidirectional cover of femoral head. The technique is relatively easy to perform. All the patients have achieved a pain free outcome to date

Summary. Between January 2003 and October 2004,12 patients with non tuberculous spondylodiscitis were treated by radical debridment, reconstruction and stabilization. In our group 9 patients underwent posterior procedure and 3 underwent combined anterior and posterior procedures. 3 of these had fungal and 9 pyogenic infections. All the patients had appropriate antimicrobial therapy All patients had excellent to good functional results and no evidence of infection at 2 year follow-up. Introduction. Surgical treatment of nontuberculous spon-dylodiscitis of lumbar spine is challenging due to extensive bone involvement and comorbid conditions. This study is to assess the role of radical debridment followed by reconstruction and stabilization of affected segments in reducing morbidity and mortality in these patients. Methods. 12 consecutive patients were operated between January 2003 and October 2004. Patients presented with severe back pain, root compression or paraparesis.7 cases had prior spinal surgery. Blood and radiological investigations were diagnostic. All these patients underwent radical debridement, reconstruction and stabilization of affected segments done with titanium pedicular screws, titanium mesh cages, cancellous iliac crest graft. Only posterior procedure in 9 cases, combined anterior posterior in 3 cases followed by adequate and appropriate antimicrobials therapy. Follow-up ranged from 25 to 35 months. Results: 3 cases were fungal and 9 were pyogenic infection. Oswestry low back questionnaire, kirkaldy-willis criteria showed dramatic improvement of function. All the blood parameters were normalized in 3 months.1 case had dural tear which was repaired immediately,3 cases had wound exploration and lavage. No major complications were encountered. All cases showed Radiological fusion at last follow-up. Discussion: Radical debridement of necrotic material, decompression of neurological structures, create a good vascularised environment. Restoring stability compromised by either infection or prior surgery helps in healing process and reduces morbidity of patients. Significance: Reconstruction using pedicular system and interbody devices can safely be used in presence of non tuberculous infection provided debridement has been radical

Aims: Bone graft substitutes have been advocated recently to avoid the potential morbidity associated with harvest of autogenous iliac crest graft. However, no commercially available graft equals autogenous bone osteo-inductive and osteoconductive qualities. We reviewed our patientsƱ morbidity after harvest of 240 anterior iliac crest bone grafts for procedures involving the foot and ankle. Methods: A computerized analysis of patient records was undertaken to identify all patients under-all going a unique unicortical iliac crest bone graft harvest over a 10-year period from the senior authorƱs practice. All patients were contacted either by phone or mailed questionnaire inquiring about the postoperative morbidity of the procedure. Charts were reviewed for any related complications. Results: 200 patients were available for follow-up (range 1 to 10 years). Of these patients 98% were satisĆ¾ed or very satisĆ¾ed with their bone graft harvest. 10 patients complained of minor residual numbness lateral to the harvest site. None complained of problems with ambulation related to graft site pain. 7% reported that their graft site pain was greater than their operative site pain during the postoperative period. 95% of patientsƱ pain resolved within 4 weeks of the operative procedure. No patients incurred extra hospital days as a result of the bone graft harvest. No deep infections occurred, although there was a 6% incidence of postoperative hematoma/seroma. Conclusions: Despite common sentiment, harvesting of autogenous iliac crest bone graft yields minimal morbidity, no extra hospitalization, and optimal bone graft material at similar or lesser cost than bone graft substitutes while being overwhelmingly acceptable to patients. Bone graft substitutes may not be as cost-effective as currently thought

A similar scale was used to score the extent of fusion. Results: As shown in Table 1, both low and high current stimulation groups had generally increased incidence of bony fusion compared to the non-stimulated and femoral allograft ring groups. At 26 weeks, the extent of bony fusion increased with the devices from 43% to 75% in a dose-dependent fashion, compared to 25% with the femoral rings. Mechanical testing also demonstrated similar increases in mechanical stiffness in a dose-dependent fashion. Introduction and Aims: Non-union following triple arthrodesis has been significant, up to 23%. Iliac crest bone grafting and internal fixation has reduced this to 0ā€“4%. Harvesting bone graft incurs significant donor site morbidity and may be unnecessary. We present the results of 100 triple arthrodeses performed with local graft, avoiding donor site complications. Method: Between January 1993 and July 2002 the senior author performed 112 triple arthrodeses. We performed a retrospective chart review, evaluating the incidence of union and the post-operative complications, with a minimum follow-up of six months. For this study, we excluded all (seven) revision fusions, and all (five) fusions using iliac crest or other donor site graft (e.g. calcaneus or tibia). One hundred fusions were performed using a standard technique with internal fixation and without supplementary bone graft. All patients were reviewed until fusion had occurred or non-union declared. Results: Ninety-six patients had one hundred triple arthrodeses, with an average age of 58 years. Fifty fusions were performed for posterior tibial tendon deficiency, 20 for rheumatoid arthritis, seven for osteoarthritis and 12 for post-traumatic osteoarthritis. The remaining 11 cases included: tarsal coalition, psoriatic arthritis and polio. There were 26 males and 74 females, with 37 fusions performed on the right foot and 73 on the left. The average time to union was 5.4 months (range 3ā€“24) with three patients developing non-unions. Of the three patients diagnosed with a non-union, two had fair outcomes and elected to be treated non-operatively. The third non-union had a poor result, and achieved a successful outcome following revision. Overall, there were 74 good outcomes, 22 fair and four poor results. The complications following surgery included: nine wound infections, one DVT, one malunion and seven cases of prominent screws, requiring removal. The one patient with malunion was successfully revised at two years. Conclusion: The majority of authors advocate the use of iliac crest bone graft during triple arthrodesis. However, harvesting iliac crest graft has a significant short and long-term morbidity. This study indicates that comparable rates of union are achieved without the need for supplementary bone graft and thereby avoiding donor site morbidity

Aims

We present our experience of using a metal-backed prosthesis and autologous bone graft to treat gross glenoid bone deficiency.

Patients and Methods

A prospective cohort study of the first 45 shoulder arthroplasties using the SMR Axioma Trabecular Titanium (TT) metal-backed glenoid with autologous bone graft. Between May 2013 and December 2014, 45 shoulder arthroplasties were carried out in 44 patients with a mean age of 64 years (35 to 89). The indications were 23 complex primary arthroplasties, 12 to revise a hemiarthroplasty or resurfacing, five for aseptic loosening of the glenoid, and five for infection.

Introduction: Minimally-invasive techniques are being advocated increasingly for spine surgery, on the grounds that they are less traumatic and reduce postoperative recovery time. A minimally-invasive technique for posterior lumbar interbody fusion (MPLIF) has become available. In order to compare its efficiency and effectiveness with the open technique (OPLIF), a prospective audit was undertaken. Methods: Forty-seven adult patients with radicular pain resulting from isthmic or degenerative spondylolisthesis, with a slip less than 50%, and no previous surgery, elected to undergo either MPLIF (n = 23) or OPLIF (n = 24). The MPLIF procedure was performed through two, 2.5cm paramedian incisions using a tubular retractor system and dilators (METRx-MD, Medtronic Sofamor Danek, Memphis). OPLIF was performed thorugh a 10 cm midline incision. In both procedures, the listhesis was reduced using polyaxial pedicle screws, and reduction tabs or bolts (Expedium, Depuy); the disc space was distracted using interbody spacers (R-90, Metronic Sofamor Danek) and packed with iliac crest graft. Intra-operative and postoperative variables were recorded. Clinical outcomes were assessed, before and 12 months after surgery, using a visual analog scale for pain and the SF36 for function. Results: The two groups were comparable, demographically and with rspect to clinical features at inception, save that the MPLIF group had signficantly greater disc heights. Listhesis (median; interquartile range) was reduced from 25% (20ā€“32) to 8% (1ā€“13) after OPLIF, and from 20% (15ā€“25) to 5% (0ā€“10) after MPLIF. Disc height was increased from 12% (6ā€“17) of vertebral body height to 24% (20ā€“26) after OPLIF, from 17% (10ā€“23) to 30% (26ā€“36) after MPLIF. Fusion was achieved in all patients except one in the PLIF group. After OPLIF, median scores (interquartile ranges) for leg pain reduced from 8 (7ā€“9) to 1 (0ā€“4); scores for back pain reduced from 8 (6ā€“8) to 2 (1ā€“4); social functioning improved from 38 (13ā€“57) to 82 (47ā€“100), and in physical functioning improved from 20 (5ā€“48) to 68 (44ā€“86). After MPLIF, leg pain reduced from 8 (7ā€“9) to 1 (0ā€“3); back pain reduced from 8 (8ā€“10) to 2 (1ā€“4); social functioning improved from 38 (25ā€“66) to 75 (50ā€“91), and physical functioning improved from 20 (10ā€“48) to 65 (34ā€“82). All improvements within groups were significant (p = 0.000), but no statistically significant differences occurred between the groups for any outcome measure. Improvements in leg pain amounted to an 88% reduction for both groups, Back pain improved by 64% after OPLIF and by 78% after MPLIF. Duration of surgery and need for transfusion were not different between groups (5 patients required transfusion during OPLIF, and 1 during MPLIF (p = 0.09)); but the MPLIF patients had significantly shorter delays before commencing and achieving mobilization post-operatively, and had a shorter length of stay (4 days v 7 days). Discussion: Clinical outcomes after MPLIF and OPLIF were not statistically different. Both procedures reduced back pain as well as leg pain, and restored function. Although there was no detectable correlation between pain and function before treatment, relief of pain was strongly correlated with restoration of function, after treatment. The advantage of MPLIF is that promotes faster recovery and shortens hospital stay. Its only disadvantage is the need to adapt to the technology involved and becoming familiar and confident with its use

Purpose of the study. Two patients with very severe thoracolumbar Scheuermann's kyphosis who developed spontaneous bony fusion across the apex of the deformity are presented and their treatment, as well as surgical outcome is discussed. Summary of Background Data. Considerable debate exists regarding the pathogenesis, natural history and treatment of Scheuermann's kyphosis. Surgical correction is indicated in the presence of severe kyphosis which carries the risk of neurological complications, persistent back pain and significant cosmetic deformity. Methods. We reviewed the medical notes and radiographs of 2 adolescent patients with severe thoracolumbar Scheuermann's kyphosis who developed spontaneous posterior and anteroposterior fusion across the apex of the deformity. Results. Patient 1. A male patient aged 17 years and 11 months underwent kyphosis correction when the deformity measured 115o and only corrected to 100o on supine hyperextension radiograph against the bolster; he had a small associated scoliosis. The surgery involved a combined single-stage anterior and posterior spinal arthrodesis T4-L3 with the use of posterior pedicle hook/screw/rod instrumentation and autologous rib graft. The anterior longitudinal ligament was ossified from T10 to L1 with bridging osteophytes extending circumferentially from T11 to T12 at the apex of kyphosis and displacing the major vessels anteriorly. The intervertebral discs from T9 to T12 were very stenotic and immobile. The osteophytes were excised both on the convexity and concavity of the associated thoracolumbar scoliosis. The anterior longitudinal ligament was released and complete discectomies back to the posterior longitudinal ligament were performed from T7 to L1. During the posterior exposure, the spine was found to be spontaneously fused across the apex of the kyphosis from T9 to L1. There were no congenital vertebral anomalies. Extensive posterior apical closing wedge osteotomies were performed from T7 to T12. The fused facets and ossified ligamentum flavum were excised and the spine was mobilised at completion of the anterior and posterior osteotomies. The kyphosis was corrected using a cantilever maneuver from proximal to distal under spinal cord monitoring. Excellent correction to 58o was achieved and maintained at follow-up. Autologous rib graft was used to enhance a solid bony fusion. Patient 2. A female patient aged 18 years and one month underwent kyphosis correction when the deformity measured 115o and only corrected to 86o on supine hyperextension radiograph against the bolster; she had a small thoracolumbar scoliosis. The surgery involved a single-stage posterior spinal arthrodesis T2-L4 with the use of posterior pedicle hook/screw/rod instrumentation and autologous iliac crest bone. The spine was spontaneously fused across the apex of kyphosis from T9 to L1. There were no congenital vertebral anomalies. Extensive posterior apical closing wedge osteotomies were performed from T6 to T12. The fused facets and ossified ligamentum flavum were excised and the spine was mobilised at completion of the osteotomies. The kyphosis was corrected using a cantilever maneuver from proximal to distal under spinal cord monitoring. Excellent correction to 60o was achieved and maintained at follow-up. Autologous iliac crest graft was used to achieve a solid bony fusion. In both patients the preoperative MRI assessed the intraspinal structures but failed to diagnose the solid fusion across the posterior bony elements at the apex of kyphosis. A CT scan with 3D reconstruction would have illustrated the bony anatomy across the kyphosis giving valuable information to assist surgical planning. This is recommended in the presence of rigid thoracolumbar Scheuermann's kyphosis which does not correct in hyperextension, especially if the plain radiograph shows anterior bridging osteophytes. Conclusion. Spontaneous posterior or anteroposterior fusion can occur across the apex of severe thoracolumbar Scheuermann's kyphosis; this should be taken into account when surgical correction is anticipated. The bony ankylosis may represent the natural history of an extreme deformity as an attempt of the spine to auto-stabilise. A combination of factors including a rigid deformity, which limits significantly active movement of the spine, as well as anterior vertebral body wedging with severe adjacent disc stenosis which induces bridging osteophyte formation may result in the development of spontaneous fusion across the apex of the kyphosis either posteriorly or anteroposteriorly. In the presence of an isolated posterior fusion, segmental posterior closing wedge osteotomies with complete excision of the ossified ligamentum flavum and fused facets should mobilise the thoracolumbar spine and allow for kyphosis correction. An additional anterior spinal release including complete discectomies, resection of the anterior longitudinal ligament and osteophytes is required if the bony fusion extends anteroposteriorly. Patients with Scheuermann's kyphosis should be ideally treated at an earlier stage and with a lesser degree of deformity so that this ossification process is prevented


Aims

The aim of this study was to evaluate the outcomes of a salvage procedure using a 95Ā° angled blade plate for failed osteosynthesis of atypical subtrochanteric femoral fractures associated with the long-term use of bisphosphonates. These were compared with those for failed osteosynthesis of subtrochanteric fractures not associated with bisphosphonate treatment.

Patients and Methods

Between October 2008 and July 2016, 14 patients with failed osteosynthesis of an atypical subtrochanteric femoral fracture were treated with a blade plate (atypical group). Their mean age was 67.8 years (60 to 74); all were female. During the same period, 21 patients with failed osteosynthesis of a typical subtrochanteric fracture underwent restabilization using a blade plate (typical group). Outcome variables included the time of union, postoperative complications, Harris Hip Score, and Sanders functional rating scale.


We evaluated the outcome of treatment of nonunion of an intracapsular fracture of the femoral neck in young patients using two cannulated screws and a vascularised bone graft. A total of 32 patients (15 women and 17 men, with a mean age of 36.5 years; 20 to 50) with failed internal fixation of an intracapsular fracture were included in the study. Following removal of the primary fixation, two cannulated compression screws were inserted with a vascularised iliac crest bone graft based on the ascending branch of the lateral femoral circumflex artery.

At a mean follow-up of 6.8 years (4 to 10), union was achieved in 27 hips (84%). A total of five patients with a mean age of 40.5 years (35 to 50) had a persistent nonunion and underwent total hip arthroplasty as also did two patients whose fracture united but who developed osteonecrosis of the femoral head two years post-operatively. Statistical analysis showed that younger patients achieved earlier and more reliable union (p < 0.001). The functional outcome, as assessed by the Harris Hip score, was better in patients aged < 45 years compared with those aged > 45 years (p < 0.001).

These findings suggest that further fixation using two cannulated compression screws and a vascularised iliac crest bone graft is an effective salvage treatment in patients aged < Ā 45 years, in whom osteosynthesis of a displaced intracapsular fractures of the femoral neck has failed.

We hypothesised that cells obtained via a Reamerā€“Irrigatorā€“Aspirator (RIA) system retain substantial osteogenic potential and are at least equivalent to graft harvested from the iliac crest. Graft was harvested using the RIA in 25 patients (mean age 37.6 years (18 to 68)) and from the iliac crest in 21 patients (mean age 44.6 years (24 to 78)), after which ā‰„ 1Ā g of bony particulate graft material was processed from each. Initial cell viability was assessed using Trypan blue exclusion, and initial fluorescence-activated cell sorting (FACS) analysis for cell lineage was performed. After culturing the cells, repeat FACS analysis for cell lineage was performed and enzyme-linked immunosorbent assay (ELISA) for osteocalcin, and Alizarin red staining to determine osteogenic potential. Cells obtained via RIA or from the iliac crest were viable and matured into mesenchymal stem cells, as shown by staining for the specific mesenchymal antigens CD90 and CD105. For samples from both RIA and the iliac crest there was a statistically significant increase in bone production (both p < 0.001), as demonstrated by osteocalcin production after induction.

Medullary autograft cells harvested using RIA are viable and osteogenic. Cell viability and osteogenic potential were similar between bone grafts obtained from both the RIA system and the iliac crest.

There are few reports on the treatment of pyogenic lumbar spondylodiscitis through the posterior approach using a single incision. Between October 1999 and March 2003 we operated on 18 patients with pyogenic lumbar spondylodiscitis. All underwent posterior lumbar interbody fusion using an autogenous bone graft from the iliac crest and pedicle screws via a posterior approach. The clinical outcome was assessed using the Frankel neurological classification and the criteria of Kirkaldy-Willis. Under the Frankel classification, two patients improved by two grades (C to E), 11 by one grade, and five showed no change. The Kirkaldy-Willis functional outcome was excellent in five patients, good in ten and fair in three. Bony union was confirmed six months after surgery in 17 patients, but in one patient this was not achieved until two years after operation. The mean lordotic angle before operation was 20Ā° (āˆ’2Ā° to 42Ā°) and the mean lordotic angle at the final follow-up was 32.5Ā° (17Ā° to 44Ā°). Two patients had a superficial wound infection and two a transient root injury. Posterior lumbar interbody fusion with an autogenous iliac crest bone graft and pedicle screw fixation via a posterior approach can provide satisfactory results in pyogenic spondylodiscitis.

We studied the effects of hyperbaric oxygen (HBO) and zoledronic acid (ZA) on posterior lumbar fusion using a validated animal model. A total of 40 New Zealand white rabbits underwent posterior lumbar fusion at L5ā€“6 with autogenous iliac bone grafting. They were divided randomly into four groups as follows: group 1, control; group 2, HBO (2.4 atm for two hours daily); group 3, local ZA (20 Ī¼g of ZA mixed with bone graft); and group 4, combined HBO and local ZA. All the animals were killed six weeks after surgery and the fusion segments were subjected to radiological analysis, manual palpation, biomechanical testing and histological examination.

Five rabbits died within two weeks of operation. Thus, 35 rabbits (eight in group 1 and nine in groups 2, 3 and 4) completed the study. The rates of fusion in groups 3 and 4 (p = 0.015) were higher than in group 1 (p < 0.001) in terms of radiological analysis and in group 4 was higher than in group 1 with regard to manual palpation (p = 0.015). We found a statistically significant difference in the biomechanical analysis between groups 1 and 4 (p = 0.024). Histological examination also showed a statistically significant difference between groups 1 and 4 (p = 0.036).

Our results suggest that local ZA combined with HBO may improve the success rate in posterior lumbar spinal fusion.

This study was performed to determine whether pure cancellous bone graft and Kirschner (K-) wire fixation were sufficient to achieve bony union and restore alignment in scaphoid nonunion. A total of 65 patients who underwent cancellous bone graft and K-wire fixation were included in this study. The series included 61 men and four women with a mean age of 34 years (15 to 72) and mean delay to surgery of 28.7 months (3 to 240). The patients were divided into an unstable group (A) and stable group (B) depending on the pre-operative radiographs. Unstable nonunion was defined as a lateral intrascaphoid angle > 45Ā°, or a radiolunate angle > 10Ā°. There were 34 cases in group A and 31 cases in group B. Bony union was achieved in 30 patients (88.2%) in group A, and in 26 (83.9%) in group B (p = 0.439). Comparison of the post-operative radiographs between the two groups showed no significant differences in lateral intrascaphoid angle (p = 0.657) and scaphoid length (p = 0.670) and height (p = 0.193). The radiolunate angle was significantly different (p = 0.020) but the mean value in both groups was < 10Ā°. Comparison of the dorsiflexion and palmar flexion of movement of the wrist and the mean Mayo wrist score at the final clinical visit in each group showed no significant difference (p = 0.190, p = 0.587 and p = 0.265, respectively). Cancellous bone graft and K-wire fixation were effective in the treatment of stable and unstable scaphoid nonunion.

Structural allografts may be used to manage uncontained bone defects in revision total knee replacement (TKR). However, the availability of cadaver grafts is limited in some areas of Asia. The aim of this study was to evaluate the mid-term outcome of the use of femoral head allografts for the reconstruction of uncontained defects in revision TKR, focusing on complications related to the graft.

We retrospectively reviewed 28 patients (30 TKRs) with Anderson Orthopaedic Research Institute (AORI) type 3 bone defects, who underwent revision using femoral head allografts and stemmed components. The mean number of femoral heads used was 1.7 (1 to 3). The allograftā€“host junctions were packed with cancellous autograft.

At a mean follow-up of 76 months (38 to 136) the mean American Knee Society knee score improved from 37.2 (17 to 60) pre-operatively to 90 (83 to 100) (p < 0.001). The mean function score improved from 26.5 (0 to 50) pre-operatively to 81 (60 to 100) (p < 0.001). All the grafts healed to the host bone. The mean time to healing of the graft was 6.6 months (4 to 16). There have been no complications of collapse of the graft, nonunion, infection or implant loosening. No revision surgery was required.

The use of femoral head allografts in conjunction with a stemmed component and autogenous bone graft in revision TKR in patients with uncontained bone defects resulted in a high rate of healing of the graft with minimal complications and a satisfactory outcome. Longer follow-up is needed to observe the evolution of the graft.

Aims

We reviewed 34 consecutive patients (18 female-16 male) with isthmic spondylolysis and grade I to II lumbosacral spondylolisthesis who underwent in situ posterolateral arthodesis between the L5 transverse processes and the sacral ala with the use of iliac crest autograft. Ten patients had an associated scoliosis which required surgical correction at a later stage only in two patients with idiopathic curves unrelated to the spondylolisthesis.

Methods

No patient underwent spinal decompression or instrumentation placement. Mean surgical time was 1.5 hours (1 to 1.8) and intra-operative blood loss 200 ml (150 to 340). There was one wound infection treated with antibiotics but no other complication. Radiological assessment included standing posteroanterior and lateral, Ferguson and lateral flexion/extension views, as well as CT scans.

We reviewed 13 patients with infected nonunion of the distal femur and bone loss, who had been treated by radical surgical debridement and the application of an Ilizarov external fixator. All had severely restricted movement of the knee and a mean of 3.1 previous operations. The mean length of the bony defect was 8.3 cm and no patient was able to bear weight.

The mean external fixation time was 309.8 days. According to Paleyā€™s grading system, eight patients had an excellent clinical and radiological result and seven excellent and good functional results. Bony union, the ability to bear weight fully, and resolution of the infection were achieved in all the patients. The external fixation time was increased when the definitive treatment started six months or more after the initial trauma, the patient had been subjected to more than four previous operations and the initial operation had been open reduction and internal fixation.

This pilot study analysed the outcome of open versus endoscopic curettage and bone grafting for the treatment of simple calcaneal bone cysts. A total of 26 patients were evaluated into two equal groups: group 1 was treated with traditional open curettage and bone grafting and group 2 was treated with endoscopic curettage and percutaneous bone grafting. Cyst size, operating time, length of stay, time to healing, complications, further surgery and radiological healing were recorded and differences were statistically compared.

The mean age of the patients was 22.9 years (18 to 28) and the mean follow-up was 28.7Ā months (24 to 36). There were no statistically significant differences in regard to age of patients, cyst size and the follow-up periods in the two groups. The operating time and mean length of stay of group 2 patients was significantly shorter than group 1 patients (pĀ < Ā 0.001). The time to healing was similar in the two groups. The overall success rates for groups 1 and 2 were 92.3% (12 of 13) and 100% (13 of 13), respectively, and there were no statistically significant differences regarding radiological healing. This pilot study suggests that endoscopic curettage and percutaneous grafting is a simple and safe form of treatment, with similar results to those following open treatment.

We describe a retrospective review of 38 cases of reconstruction following resection of the metaphysiodiaphysis of the lower limb for malignant bone tumours using free vascularised fibular grafts. The mean follow-up was for 7.6 years (0.4 to 18.4). The mean Musculoskeletal Tumor Society score was 27.2 (20 to 30). The score was significantly higher when the graft was carried out in a one-stage procedure after resection of the tumour rather than in two stages. Bony union was achieved in 89% of the cases. The overall mean time to union was 1.7 years (0.2 to 10.3).

Free vascularised fibular transfer is a major operation with frequent, but preventable, complications which allows salvage of the limb with satisfactory functional results.

Our aim was to evaluate the results of triple arthrodesis, performed without the use of supplementary bone graft. We carried out a retrospective review of 100 consecutive triple arthrodeses. All the operations had been performed by the senior author (TSS) using a standard technique. Only local bone graft from the excised joint surfaces had been used, thereby avoiding complications at the donor site.

The mean age of the patients at surgery was 58 years (18 to 84). The mean time to union was 5.1 months (3 to 17). There were 75 good, 20 fair and five poor results. There were four cases of nonunion.

Our study has shown that comparable rates of union are achieved without the need for supplementary bone graft from the iliac crest or other donor site.

We reviewed 212 consecutive patients with adolescent idiopathic scoliosis who underwent posterior spinal arthrodesis using all pedicle screw instrumentation in terms of clinical, radiological and Scoliosis Research Society (SRS)-22 outcomes. In Group 1 (51 patients), the correction was performed over two rods using bilateral segmental pedicle screws. In GroupĀ 2 (161 patients), the correction was performed over one rod using unilateral segmental pedicle screws with the second rod providing stability of the construct through two-level screw fixation at proximal and distal ends. The mean age at surgery was 14.8 years in both groups. Comparison between groups showed no significant differences with regard to age and Risser grade at surgery, pre- and post-operative scoliosis angle, coronal Cobb correction, length of hospital stay and SRS scores. Correction of upper thoracic curves was significantly better in Group 1 (p = 0.02). Increased surgical time and intra-operative blood loss was recorded in Group 1 (p < 0.001 and p = 0.04, respectively). The implant cost was reduced by mean 35% in Group 2 due to the lesser number of pedicle screws.

Unilateral and bilateral pedicle screw techniques have both achieved excellent deformity correction in adolescent patients with idiopathic scoliosis, which was maintained at two-year follow-up. This has been associated with high patient satisfaction and low complication rates.

Dislocation of the shoulder may occur during seizures in epileptics and other patients who have convulsions. Following the initial injury, recurrent instability is common owing to a tendency to develop large bony abnormalities of the humeral head and glenoid and a susceptibility to further seizures. Assessment is difficult and diagnosis may be missed, resulting in chronic locked dislocations with protracted morbidity. Many patients have medical comorbidities, and successful treatment requires a multidisciplinary approach addressing the underlying seizure disorder in addition to the shoulder pathology. The use of bony augmentation procedures may have improved the outcomes after surgical intervention, but currently there is no evidence-based consensus to guide treatment. This review outlines the epidemiology and pathoanatomy of seizure-related instability, summarising the currently-favoured options for treatment, and their results.

There is little information about the management of peri-prosthetic fracture of the humerus after total shoulder replacement (TSR). This is a retrospective review of 22 patients who underwent a revision of their original shoulder replacement for peri-prosthetic fracture of the humerus with bone loss and/or loose components. There were 20 women and two men with a mean age of 75 years (61 to 90) and a mean follow-up 42 months (12 to 91): 16 of these had undergone a previous revision TSR. Of the 22 patients, 12 were treated with a long-stemmed humeral component that bypassed the fracture. All their fractures united after a mean of 27 weeks (13 to 94). Eight patients underwent resection of the proximal humerus with endoprosthetic replacement to the level of the fracture. Two patients were managed with a clam-shell prosthesis that retained the original components. The mean Oxford shoulder score (OSS) of the original TSRs before peri-prosthetic fracture was 33 (14 to 48). The mean OSS after revision for fracture was 25 (9 to 31). Kaplan-Meier survival using re-intervention for any reason as the endpoint was 91% (95% confidence interval (CI) 68 to 98) and 60% (95% CI 30 to 80) at one and five years, respectively.

There were two revisions for dislocation of the humeral head, one open reduction for modular humeral component dissociation, one internal fixation for nonunion, one trimming of a prominent screw and one re-cementation for aseptic loosening complicated by infection, ultimately requiring excision arthroplasty. Two patients sustained nerve palsies.

Revision TSR after a peri-prosthetic humeral fracture associated with bone loss and/or loose components is a salvage procedure that can provide a stable platform for elbow and hand function. Good rates of union can be achieved using a stem that bypasses the fracture. There is a high rate of complications and function is not as good as with the original replacement.

We report the use of a free vascularised iliac bone graft in the treatment of 21 patients (19 men and 2 women) with an avascular nonunion of the scaphoid in which conventional bone grafting had previously failed. The mean age of the patients was 32 years (23 to 46) and the dominant wrist was affected in 14. The mean interval from fracture to the vascularised bone grafting was 39 months (9 to 62). Pre-operative MRI showed no contrast enhancement in the proximal fragment in any patient. Fracture union was assessed radiologically or with CT scans if the radiological appearances were inconclusive.

At a mean follow-up of 5.6 years (2 to 11) union was obtained in 16 patients. The remaining five patients with a persistent nonunion continued to experience pain, reduced grip strength and limited range of wrist movement. In the successfully treated patients the grip strength and range of movement did not recover to match the uninjured side.

Prevention of progressive carpal collapse, the absence of donor site morbidity, good subjective results and pain relief, justifies this procedure in the treatment of recalcitrant nonunion of the scaphoid.

Glenoid replacement is technically challenging. Removal of a cemented glenoid component often results in a large osseous defect which makes the immediate introduction of a revision prosthesis almost impossible. We describe a two-stage revision procedure using a reversed shoulder prosthesis. Freeze-dried allograft with platelet-derived growth factor was used to fill the glenoid defect. Radiological incorporation of the allograft was seen and its consistency allowed the placement of a screwed glenoid component. There were no signs of new mature bone formation on histological examination.

The addition of platelet-derived growth factor to the allograft seems to contribute to an increase in incorporation and hardness, but does not promote the growth of new bone.

Despite advances in reconstructive surgery, salvage of mangled extremities still requires long periods of treatment with many operations that can be taxing both to the surgeon and the patient. Attempts at reconstruction of severely shattered limbs necessitate counselling with regard to the protracted course of treatment and associated morbidity as well as problems which may require abandoning of the procedure and secondary amputation. We report the successful salvage of a severely comminuted and open fracture of the tibia in a 32-year-old man.

Advances in the design of the components for total ankle replacement have led to a resurgence of interest in this procedure.

Between January 1999 and December 2004, 16 patients with a failed total ankle replacement were referred to our unit. In the presence of infection, a two-stage salvage procedure was planned. The first involved the removal of the components and the insertion of a cement spacer. Definitive treatment options included hindfoot fusion with a circular frame or amputation. When there was no infection, a one-stage salvage procedure was planned. Options included hindfoot fusion with an intramedullary nail or revision total ankle replacement. When there was suspicion of infection, a percutaneous biopsy was performed. The patients were followed up for a minimum of 12 months.

Of the 16 patients, 14 had aseptic loosening, five of whom underwent a revision total ankle replacement and nine a hindfoot fusion. Of the two with infection, one underwent fusion and the other a below-knee amputation. There were no cases of wound breakdown, nonunion or malunion.

Management of the failed total ankle replacement should be performed by experienced surgeons and ideally in units where multidisciplinary support is available. Currently, a hindfoot fusion appears to be preferable to a revision total ankle replacement.
