The purpose of our study was to evaluate the migration pattern of a cementless acetabular component for a metal on metal hip resurfacing. Method: Between January 2006 and June 2007, 130 patients underwent metal on metal hip resurfacing; 66 hips in 60 patients were included in this analysis. Forty-eight patients were male and 12 were female, with a mean age of 50 (range, 32–66). Ninety-five percent of the surgeries were performed for osteoarthritis. All surgeries were performed by two surgeons using the Conserve Plus (Wright Medical Technology, Memphis, TN) hip resurfacing system. The acetabular component is a monoblock cobalt chrome with a porous beaded surface for osteointegration. In all cases acetabular migration was measured both vertically and horizontally, on serial radiographs using the computer-assisted Ein Bild Röntgen Analyse (EBRA) method. A minimum of three comparable radiographs is necessary for calculating the migration curves. We scored medial migration as negative horizontal movement. Results: At a mean follow up 25.3 months (range, 24–36 months), each hip had an average of 5.1 radiographs for analysis. The software excluded two cases for poor comparability ending finally with 64 cases for the analysis. Eighty-seven point five per cent of the cups showed less than 1 mm migration in the medium-lateral axis and 54.7% less than 1 mm in the vertical axis. Surgeons generally perform total knee replacement using either a gap balancing or measured resection approach. In gap balancing, ligamentous releases are performed first to create an equal joint space before any bony resections are performed. In measured resection, bony resections are performed first to match anatomical landmarks, and soft tissue releases are subsequently performed to balance the joint space. Previous studies have found a greater rate of coronal instability and femoral component lift-off using the measured resection technique, but it is unknown how potential differences in loading translate into component stability and fixation. Patients were randomly assigned at the time of referral to a surgeon performing either the gap balancing or measured resection technique (n = 12 knees per group). Both groups received an identical cemented, posterior-stabilized implant. At the time of surgery, marker beads were inserted in the bone around the implants to enable radiostereometeric analysis (RSA) imaging. Patients underwent supine RSA exams at 0–2 weeks, 6 weeks, 3 months, 6 months, and 12 months. Migration of the tibial and femoral components including maximum total point motion (MTPM) was calculated using model-based RSA software. We conducted a systematic review of studies evaluating surrogate markers for predicting long-term outcome in primary THA. Long-term outcome was defined as revision rate of an implant at ten years according to National Institute of Health and Care Excellence guidelines. We conducted a search of Medline and Embase (OVID) databases. Separate search strategies were devised for the Cochrane database and Google Scholar. Each search was performed to include articles from the date of their inception to June 8, 2015. Results. Our search strategy identified 1082 studies of which 115 studies were included for full article review. Following review, 17 articles were found that investigated surrogate markers of long-term outcome. Introduction. Revision total hip arthroplasty is often associated with acetabular bone defects. In most cases, assessment of such defects is still qualitative and biased by subjective interpretations. Three-dimensional imaging techniques and novel anatomical reconstructions using statistical shape models (SSM) allow a more impartial and quantitative assessment of acetabular bone defects [1]. The objectives of this study are to define five clinically relevant parameters and to assess 50 acetabular bone defects in a quantitative way. Methods. Anonymized CT-data of 50 hemi-pelvises with acetabular bone defects were included in the study. The assessment was based on solid models of the defect pelvis (i.e. pelvis with bone defect) and its anatomical reconstruction (i.e. native pelvis without bone defect) (Fig.1A). Five clinically relevant parameters were defined: (1) Bone loss, defined by subtracting defect pelvis from native pelvis. (2) Bone formation, defined by subtracting native pelvis from defect pelvis. Bone formation represents bone structures, which were not present in the native pelvis (e.g. caused by remodeling processes around a migrated implant). The trabecular metal Monoblock TKR is comprised of a porous tantalum base plate with the polyethylene liner embedded directly in the porous metal. An alternative design, the trabecular metal Modular TKR, allows polyethylene liner insertion into the locking base plate after base plate implantation, but removes the low modulus of elasticity that was inherent in the Monoblock design. The purpose of this study was to compare the fixation of the Monoblock and Modular trabeucular metal base plates in a randomised controlled trial. Fifty subjects (30 female) were randomly assigned to receive the uncemented trabecular metal Monoblock or uncemented trabecular metal Modular knee replacement. A standard procedure of tantalum marker insertion in the proximal tibial and polyethylene liner was followed with uniplanar radiostereometric analysis (RSA) examinations immediately post-operatively and at 6 week, 3 month, 6 month, and 12 month follow-ups. Modern metal-on-metal (MoM) hip resurfacing arthroplasty (HRA), while achieving good results with well-orientated, well-designed components in ideal patients, is contraindicated in women, men with head size under 50 mm, or metal hypersensitivity. These patients currently have no access to the benefits of HRA. Highly crosslinked polyethylene (XLPE) has demonstrated clinical success in total hip arthroplasty (THA) and, when used in HRA, potentially reduces metal ion-related sequelae. We report the early performance of HRA using a direct-to-bone cementless mono-bloc XLPE component coupled with a cobalt-chrome femoral head, in the patient group for whom HRA is currently contraindicated. Methods. This is a cross-sectional, observational assessment of 88 consecutive metal-on-XLPE HRAs performed in 84 patients between 2015 and 2018 in three centres (three surgeons, including the designer surgeon). Mean follow-up is 1.6 years (0.7 to 3.9). Mean age at operation was 56 years (. sd. 11; 21 to 82), and 73% of implantations were in female patients. All patients were individually counselled, and a detailed informed consent was obtained prior to operation. Primary resurfacing was carried out in 85 hips, and three cases involved revision of previous MoM HRA. Clinical, radiological, and Oxford Hip Score (OHS) assessments were studied, along with implant survival. Results. There was no loss to follow-up and no actual or impending revision or reoperation. Median OHS increased from 24 (interquartile range (IQR) 20 to 28) preoperatively to 48 (IQR 46 to 48) at the latest follow-up (48 being the best possible score). Objectives. Porous metal surfaces have been a popular option for acetabular component fixation in total hip arthroplasty (THA). New THA component designs are introduced periodically with the expectation of better wear properties and survivorship. Since its approval for use in 2002 there have been few clinical outcome studies published on the Pinnacle acetabular cup system. We hypothesised that the hemispherical porous coated Pinnacle acetabular cup system with a range of cup options and bearing surfaces would give us predictably good fixation and survivorship at five years post implantation. Materials and Methods. A total of 1391 Pinnacle acetabular cups (De Puy, a Johnson & Johnson company, Warsaw IN) were implanted between the period March 2003 to August 2011 by four senior surgeons. There were a total of 29 patients requiring revision surgery. Of these revisions, 23 were excluded from the final analysis. Sixteen were for early infection requiring debridement of the hip joint and exchange of the modular liner and femoral head. Five patients sustained femoral peri-prosthetic fractures requiring further surgery with retention of the acetabular component in all cases. Of the remaining 8 revision cases, only 6 had more than two years follow-up. All patients had pre and post-operative Harris hip scores, WOMAC and SF-12 scores. All patients were assessed with serial radiographs immediately post-operatively, 3 months, 1 year, 2 years, every 2 years thereafter.

Instability is a common cause of failure after total hip arthroplasty. A novel reverse total hip has been developed, with a femoral cup and acetabular ball, creating enhanced mechanical stability. The purpose of this study was to assess the implant fixation using radiostereometric analysis (RSA), and the clinical safety and efficacy of this novel design. Patients with end-stage osteoarthritis were enrolled in a prospective cohort at a single centre. The cohort consisted of 11 females and 11 males with mean age of 70.6 years (SD 3.5) and BMI of 31.0 kg/m2 (SD 5.7). The primary aim of this trial was to compare the subsidence of two similar hydroxyapatite-coated titanium femoral components from different manufacturers. Secondary aims were to compare rotational migration (anteversion/retroversion and varus/valgus tilt) and patient-reported outcome measures between both femoral components. Surgical techniques for implant alignment in total knee arthroplasty (TKA) is a expanding field as manufacturers introduce patient-specific cutting blocks derived from 3D reconstructions of pre-operative imaging, commonly MRI or CT. The patient-specific OtisMed system uses a detailed MRI scan of the knee for 3D reconstruction to estimate the kinematic axis, dictating the cutting planes in the custom-fit cutting blocks machined for each patient. The resulting planned alignment can vary greatly from a neutral mechanical axis. The purpose of this study was to evaluate the early fixation of components in subjects randomized to receive shape match derived kinematic alignment or conventional alignment using computer navigation. A subset of subjects were evaluated with gait analysis. Methods. Fifty-one patients were randomized to receive a cruciate retaining cemented total knees (Triathlon, Stryker) using computer navigation aiming for neutral mechanical axis (standard of care) or patient-specific cutting blocks (OtisMed custom-fit blocks, Stryker). Pre-operatively, all subjects had MRI scans for cutting block construction to maintain blinding. RSA exams and health outcome questionnaires were performed post-operatively at 6 week, 3, 6, and 12 month follow-ups. A subset (9 subjects) of the patient-specific group underwent gait analysis (Optotrak TM 3020, AMTI force platforms) one-year post-TKA, capturing three dimensional (3D) knee joint angles and kinematics. Principal component analysis (PCA) was applied to the 3D gait angles and moments of the patient-specific group, a case-matched control group, and 60 previously collected asymptomatic subjects. Results. Five MRI scans for surgical planning were not useable due to motion artifacts, with 2 successfully rescanned. Ligament releases were performed in 62% of navigation cases and 32% of patient-specific cases. One patient-specific case was revised for failure of the cruciate ligament, resulting in a polyethylene liner exchange for a thicker, cruciate substituting insert.

The Exeter short stem was designed for patients with Dorr type A femora and short-term results are promising. The aim of this study was to evaluate the minimum five-year stem migration pattern of Exeter short stems in comparison with Exeter standard stems. In this case-control study, 25 patients (22 female) at mean age of 78 years (70 to 89) received cemented Exeter short stem (case group). Cases were selected based on Dorr type A femora and matched first by Dorr type A and then age to a control cohort of 21 patients (11 female) at mean age of 74 years (70 to 89) who received with cemented Exeter standard stems (control group). Preoperatively, all patients had primary hip osteoarthritis and no osteoporosis as confirmed by dual X-ray absorptiometry scanning. Patients were followed with radiostereometry for evaluation of stem migration (primary endpoint), evaluation of cement quality, and Oxford Hip Score. Methods
The mean age of patients undergoing total knee arthroplasty (TKA) has reduced with time. Younger patients have increased expectations following TKA. Aseptic loosening of the tibial component is the most common cause of failure of TKA in the UK. Interest in cementless TKA has re-emerged due to its encouraging results in the younger patient population. We review a large series of tantalum trabecular metal cementless implants in patients who are at the highest risk of revision surgery. A total of 454 consecutive patients who underwent cementless TKA between August 2004 and December 2021 were reviewed. The mean follow-up was ten years. Plain radiographs were analyzed for radiolucent lines. Patients who underwent revision TKA were recorded, and the cause for revision was determined. Data from the National Joint Registry for England, Wales, Northern Island, the Isle of Man and the States of Guernsey (NJR) were compared with our series.Aims

Thresholds of acceptable early migration of the components in total knee arthroplasty (TKA) have traditionally ignored the effects of patient and implant factors that may influence migration. The aim of this study was to determine which of these factors are associated with overall longitudinal migration of well-fixed tibial components following TKA. Radiostereometric analysis (RSA) data over a two-year period were available for 419 successful primary TKAs (267 cemented and 152 uncemented in 257 female and 162 male patients). Introduction: Tibial augments are required to replace bone loss in the proximal tibia during revision total knee replacement and can be either a rectangular ‘block’ shape or an angled ‘wedge’ shape. Purpose of study: We have identified a high failure rate with the ‘wedge’ shape currently used in the Co-ordinate (DePuy) revision knee system. Methods: We report a series of revision total knee replacement procedures performed using the Co-ordinate system. All revision ‘angled’ tibial wedge implants were selected from our prospective database containing follow up records for knee replacements. Patients underwent clinical and radiological review at three months following the procedure, on the anniversary of surgery and then on a yearly basis. Those with fewer than 12 months follow-up were excluded. Results: Between December 1993 and February 2006, a total of 34 patients (19 female, 15 male) required revision knee arthroplasty with an ‘angled’ wedged tibial augment. Mean age at surgery was 59.3 years (range 46–93). Five cases were excluded due to absence of follow-up (four requiring further surgery for infection and one due to unrelated medical complications). Of the remaining 29 cases, 16 (55.2%) had no problems with the tibial implant, while 13 (44.8%) exhibited radiological changes on review. Tibial
Introduction. Aseptic loosening is the main reason for total knee arthroplasty (TKA) failure, responsible for more than 25% of the revision procedures, with most of the problems occurring with the tibial component. While early loosening can be attributed to failure of primary fixation, late implant loosening is associated with loss of fixation secondary to bone resorption due to altered physiological load transfer to the tibial bone. Several attempts have been made to investigate these changes in bone load transfer in biomechanical simulations and bone remodeling analyses, which can be useful to provide information on the effect of patient, surgery, or design-related factors. On the other hand, these factors have also been investigated in clinical studies of radiographic changes of bone density following TKA. In this study we made an overview of the knowledge obtained from these clinical studies, which can be used to inform clinical decision making and implant design choices. Methods. A literature search was performed to identify clinical follow-up studies that monitored peri-prosthetic bone changes following TKA. Within these studies, effects of the following parameters on bone density changes were investigated: post-operative time, region of interest, alignment, body weight, systemic osteoporosis, implant design and cementation. Moreover, we investigated the effect of bone density loss on implant survival. Results. A total of 19 studies was included in this overview, with a number of included patients ranging from 12 to 7,760. Most studies used DEXA (n=16), while a few studies performed analyses on calibrated digital radiographs (n=2), or computed tomography (n=1). Postoperative follow-up varied from 9 months to 10 years. Studies consistently report the largest bone density reduction within the first postoperative year. Bone loss is mainly seen in the medial region. This has been attributed to the change in alignment following surgery, during which often the pre-operative varus knee is corrected to a more physiological alignment, resulting in a load shift towards the lateral compartment. Measurements in unoperated contralateral legs were performed in 3 cases, and two studies performed standardized DEXA measurements to provide information on systemic osteoporosis. While on the short term no changes were observed, significant negative correlations have been found between severity of osteoporosis and peri-prosthetic bone density. No clear effects of bodyweight and cementation on bone loss have been identified. Although some studies do find differences between implant types, the variation in the data makes it difficult to draw general conclusions from these findings. In this study, a baseplate with composite bone autograft and a central trabecular titanium peg was implanted, and its migration was assessed for two years postoperatively using radiostereometric analysis (RSA). A total of 14 patients who underwent a rTSA with an autograft consented to participate. Of these, 11 had a primary rTSA using humeral head autograft and three had a revision rTSA with autograft harvested from the iliac crest. The mean age of the patients was 66 years (39 to 81). Tantalum beads were implanted in the scapula around the glenoid. RSA imaging (stereographic radiographs) was undertaken immediately postoperatively and at three, six, 12, and 24 months. Analysis was completed using model-based RSA software. Outcomes were collected preoperatively and at two years postoperatively, including the Oxford Shoulder Score, the American Shoulder and Elbow Score, and a visual analogue score for pain. Dislocation remains a leading cause of failure following revision total hip arthroplasty (THA). While dual-mobility (DM) bearings have been shown to mitigate this risk, options are limited when retaining or implanting an uncemented shell without modular DM options. In these circumstances, a monoblock DM cup, designed for cementing, can be cemented into an uncemented acetabular shell. The goal of this study was to describe the implant survival, complications, and radiological outcomes of this construct. We identified 64 patients (65 hips) who had a single-design cemented DM cup cemented into an uncemented acetabular shell during revision THA between 2018 and 2020 at our institution. Cups were cemented into either uncemented cups designed for liner cementing (n = 48; 74%) or retained (n = 17; 26%) acetabular components. Median outer head diameter was 42 mm. Mean age was 69 years (SD 11), mean BMI was 32 kg/m2 (SD 8), and 52% (n = 34) were female. Survival was assessed using Kaplan-Meier methods. Mean follow-up was two years (SD 0.97).Aims

Our objective was describing an algorithm to identify and prevent vascular injury in patients with intrapelvic components. Patients were defined as at risk to vascular injuries when components or cement migrated 5 mm or more beyond the ilioischial line in any of the pelvic incidences (anteroposterior and Judet view). In those patients, a serial investigation was initiated by a CT angiography, followed by a vascular surgeon evaluation. The investigation proceeded if necessary. Methods
Purpose. To report clinical results and demonstrate any posterior femoral translation (PFT) in medial rotation total knee arthroplasty (TKA) of posterior cruciate ligament (PCL) retaining type. Materials and Methods. A prospective study was performed upon thirty consecutive subjects who were operated on with medial rotation TKA of PCL retaining type (Advance® Medial Pivot prosthesis with ‘Double High’ insert; Wright Medical Technology, Arlington, TN, USA) (Fig. 1). between March 2009 and March 2010 and had been followed up for a least 2 years. Inclusion criteria were age between 60 and 75 years and primary degenerative joint disease of knee graded as Kellgren Lawrence grade III or higher. Exclusion criteria were age under 60 years, any inflammatory joint disease including rheumatoid arthritis, early stage of primary degenerative joint disease of knee or any history of previous osteotomy around knee. Clinically, the knee society knee score and function score were used to evaluate pain and function. At last follow-up, all subjects performed full extension, thirty degree flexion and full active flexion sequentially under fluoroscopic surveillance. In each of these lateral radiographs, anteroposterior(AP) condylar position was pinpointed and the magnitude of PFT was determined by degree of transition of AP condylar position from full extension to full active flexion radiograph (Fig. 2 A–B). Statistical methods used were paired t-test, Pearson correlation, Steadman rank correlation and regression analysis.
Large bone defects resulting from osteolysis, fractures, osteomyelitis, or metastases pose significant challenges in acetabular reconstruction for total hip arthroplasty. This study aimed to evaluate the survival and radiological outcomes of an acetabular reconstruction technique in patients at high risk of reconstruction failure (i.e. periprosthetic joint infection (PJI), poor bone stock, immunosuppressed patients), referred to as Hip Reconstruction In Situ with Screws and Cement (HiRISC). This involves a polyethylene liner embedded in cement-filled bone defects reinforced with screws and/or plates for enhanced fixation. A retrospective chart review of 59 consecutive acetabular reconstructions was performed by four surgeons in a single institution from 18 October 2018 to 5 January 2023. Cases were classified based on the Paprosky classification, excluding type 1 cases (n = 26) and including types 2 or 3 for analysis (n = 33). Radiological loosening was evaluated by an orthopaedic surgeon who was not the operating surgeon, by comparing the immediate postoperative radiographs with the ones at latest follow-up. Refobacin Bone Cement R and Palacos Overall, 75 patients were included in the study and 71 were available at two years postoperatively. Prior to surgery, they were randomized to one of the three combinations studied: Palacos cement with use of the Optivac mixing system, Refobacin with use of the Optivac system, and Refobacin with use of the Optipac system. Ceramic on ceramic (CoC) Total Hip Replacement revisions give specific problems and no consensus is made on the way to perform it. The aim of this study was to report the results of THA CoC revisions and to identify the specific concerns. Between April 2007 and May 2016, 50 consecutive revisions have been performed on average 2.8 years after the THA, in 46 patients (28 males, 18 females) at a mean age of 55 years. The main reasons for revision were loosening (34%), pain without loosening (20%), infection (12%), implant breakage (10%), and instability (8%). There were 41 implant revisions, five revisions without implant change (10%) and four revisions for head change (8%). Revisions were performed without bone reconstruction in 68% of hips and with a CoC couple in 83% of hips. Three patients have died and three were lost to follow up. There were six intraoperative femoral fractures, one early and two late non−recurrent dislocations. Eight hips (16%) had re−revision with implant replacement. As an alternative to external fixators, intramedullary lengthening nails (ILNs) can be employed for distraction osteogenesis. While previous studies have demonstrated that typical complications of external devices, such as soft-tissue tethering, and pin site infection can be avoided with ILNs, there is a lack of studies that exclusively investigated tibial distraction osteogenesis with motorized ILNs inserted via an antegrade approach. A total of 58 patients (median age 17 years (interquartile range (IQR) 15 to 21)) treated by unilateral tibial distraction osteogenesis for a median leg length discrepancy of 41 mm (IQR 34 to 53), and nine patients with disproportionate short stature treated by bilateral simultaneous tibial distraction osteogenesis, with magnetically controlled motorized ILNs inserted via an antegrade approach, were retrospectively analyzed. The median follow-up was 37 months (IQR 30 to 51). The Birmingham Hip Resurfacing (BHR) was introduced in 1997 to address the needs of young active patients using a historically proven large-diameter metal-on-metal (MoM) bearing. A single designer surgeon’s consecutive series of 130 patients (144 hips) was previously reported at five and ten years, reporting three and ten failures, respectively. The aim of this study was to extend the follow-up of this original cohort at 25 years. The study extends the reporting on the first consecutive 144 resurfacing procedures in 130 patients for all indications. All operations were undertaken between August 1997 and May 1998. The mean age at operation was 52.1 years (SD 9.93; 17 to 76), and included 37 female patients (28.5%). Failure was defined as revision of either component for any reason. Kaplan-Meier survival analysis was performed. Routine follow-up with serum metal ion levels, radiographs, and Oxford Hip Scores (OHSs) was undertaken.Aims

Custom triflange acetabular components (CTACs) play an important role in reconstructive orthopaedic surgery, particularly in revision total hip arthroplasty (rTHA) and pelvic tumour resection procedures. Accurate CTAC positioning is essential to successful surgical outcomes. While prior studies have explored CTAC positioning in rTHA, research focusing on tumour cases and implant flange positioning precision remains limited. Additionally, the impact of intraoperative navigation on positioning accuracy warrants further investigation. This study assesses CTAC positioning accuracy in tumour resection and rTHA cases, focusing on the differences between preoperative planning and postoperative implant positions. A multicentre observational cohort study in Australia between February 2017 and March 2021 included consecutive patients undergoing acetabular reconstruction with CTACs in rTHA (Paprosky 3A/3B defects) or tumour resection (including Enneking P2 peri-acetabular area). Aseptic loosening arises when periprosthetic bone loss results in mechanical failure at the host-implant interface, and is the main factor limiting implant survival after total hip arthroplasty (THA). The aims of this study were to determine whether genetic variation is a risk factor for loosening, explore the metabolic mechanisms of periprosthetic bone loss, and determine whether bisphosphonates may prevent bone loss and enhance implant mechanical stability after THA. In a genetic association study (. J Bone Mineral Res. 2003. ; . 18. :. 1995. –2001. ) we found that carriage of the −238A allele within the promoter region of the TNF gene was an independent risk factor for aseptic loosening. A subsequent reporter gene assay showed differential TNF gene responsiveness between the –238A and –238G alleles to polyethylene particule stimulation (Calcified Tissue Int 2003; 72: 251-273). In a cross-sectional study (J Orthop Res 2003; 214: 691–696) we found that subjects with aseptic loosening had lower bone mineral density (BMD) in the region of the femoral calcar and higher urinary excretion of cross-linked collagen breakdown products than their counterparts with fixed femoral implants. Artificial intelligence (AI)-based image analysis has the potential to automate this postoperative surveillance. The aim of this study was to prepare a scoping review to investigate how AI is being used in the analysis of radiographs following THA and TKA, and how accurate these tools are. The Embase, MEDLINE, and PubMed libraries were systematically searched to identify relevant articles. The Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for scoping reviews and Arksey and O’Malley framework were followed. The aim of this study was to compare a bicruciate-retaining (BCR) total knee arthroplasty (TKA) with a posterior cruciate-retaining (CR) TKA design in terms of kinematics, measured using fluoroscopy and stability as micromotion using radiostereometric analysis (RSA). A total of 40 patients with end-stage osteoarthritis were included in this randomized controlled trial. All patients performed a step-up and lunge task in front of a monoplane fluoroscope one year postoperatively. Femorotibial contact point (CP) locations were determined at every flexion angle and compared between the groups.
