Total shoulder arthroplasty (TSA) is an effective treatment to restore shoulder function and alleviate pain in the case of glenohumeral arthritis [1]. Stress shielding, which occurs when bone stress is reduced due to the replacement of bone with a stiffer metallic implant, causes bone resorption of up to 9% of the humeral cortical thickness following TSA [2]. Shorter length stems and smaller overall geometries may reduce stress shielding [3], however the effect of humeral head backside contact with the resection plane has not yet been fully investigated on bone stress. Therefore, the purpose of this study was to quantify the effect of humeral head contact conditions on bone stresses following TSA.
Methods
3D models of eight male left cadaveric humeri (68±6 years) were generated from CT data using MIMICS. These were then virtually prepared for reconstruction by an orthopaedic surgeon to accept a short-stem humeral implant (Exactech Equinoxe® Preserve) that was optimally sized and placed centrally in the humeral canal. The humeral head was positioned in the inferior-medial position such that contact was achieved on the medial cortex, and no contact existed on the lateral cortex. Three different humeral head backside contact conditions were investigated (Figure 1); full backside contact (FULL), contact with only the inferior-medial half of the resection (INF), and contact with only the superior-lateral half of the resection (SUP). Cortical bone was assigned an elastic modulus of 20 GPa and a Poisson's ratio of 0.3. Trabecular bone was assigned varying stiffness based on CT attenuation [4]. A joint reaction force was then applied representing 45˚ and 75˚ of abduction [5]. Changes in bone stress, as well as the expected bone response based on change in strain energy density [6] was then compared between the intact and reconstructed states.
Background
Stemless prostheses are recognized to be an effective solution for anatomic total shoulder arthroplasty (TSA) while providing bone preservation and shortest operating time. Reverse shoulder arthroplasty (RSA) with stemless has not showed the same effectiveness, as clinical and biomechanical performances strongly depend on the design. The main concern is related to stability and bone response due to the changed biomechanical conditions; few studies have analyzed these effects in anatomic designs through Finite Element Analysis (FEA), however there is currently no study analyzing the reverse configuration. Additionally, most of the studies do not consider the effect of changing the neck-shaft angle (NSA) resection of the humerus nor the proper assignment of spatial bone properties to the bone models used in the simulations. The aim of this FEA study is to analyze bone response and primary stability of the SMR Stemless prosthesis in reverse with two different NSA cuts and two different reverse angled liners, in bone models with properties assigned using a quantitative computed tomography (QCT) methodology.
Methods
Sixteen fresh-frozen cadaveric humeri were modelled using the QCT-based finite element methodology. The humeri were CT-scanned with a hydroxyapatite phantom to allow spatial bone properties assignment [Fig. 1]. Two implanted SMR stemless reverse configurations were considered for each humerus: a 150°-NSA cut with a 0° liner and a 135°-NSA cut with a 7° sloped liner [Fig. 2]. A 105° abduction loading condition was simulated on both the implanted reverse models and the intact (anatomic) humerus; load components were derived from previous dynamic biomechanical simulations on RSA implants for the implanted stemless models and from the OrthoLoad database for the intact humeri. The postoperative bone volume expected to resorb or remodel [Fig. 3a] in the implanted humeri were compared with their intact models in sixteen metaphyseal regions of interest (four 5-mm thick layers parallel to the resection and four anatomical quadrants) by means of a three-way repeated measures ANOVA followed by post hoc tests with Bonferroni correction. In order to evaluate primary stability, micromotions at the bone-Trabecular Titanium interface [Fig. 3b] were compared between the two configurations using a Wilcoxon matched-pairs signed-rank test. The significance level α was set to 0.05.
INTRODUCTION
The increasing incidence of periprosthetic femoral fractures (PFF) after total hip arthroplasty presents growing concerns due to challenges in treatment and increased mortality. PFF are often observed when the prosthesis is implanted in varus, especially with blade-type stems. To help elucidate its impact on the PFF risk, the specific research question is: What is the effect of misalignment of a blade-type stem (resulting in down-sized prosthesis) on 1)the distribution and magnitude of cortical stresses and 2)implant-bone micromotion.
METHOD
We developed two finite element models consisting of an average female femur implanted within a generic blade-type stem prosthesis, (i)in neutral alignment, and (ii)oriented in 5° of varus, coupled with corresponding down-sizing of the prosthesis. Each model consisted of 1.1million elements, while the average mesh length at the implant-bone interface was 0.4mm. Elastic moduli of 15GPa(cortex), 150MPa(trabecular bone), and 121GPa(implant), and Poisson's ratio of 0.3 were assumed. The distal end was fixed and the interface was defined as a surface-to-surface contact with friction coefficients (dynamic 0.3; static 0.4). Walking and stair-climbing were simulated by loading the joint contact and muscle forces after scaling to the subjects’ body weight. The peak von Mises stress and the average stress within the surface having 1cm diameter and the center at where the peak stress occurred at each contacting area, the interfacial micromotion along medial, lateral side were analyzed. For statistical analysis, two-tailed t-test was performed between the neutral and varus cases over four loading cycles with significance level of p<0.05.
Introduction
The patella experiences large forces and variable kinematic patterns throughout flexion which could influence function and patient satisfaction after a total knee arthroplasty (TKA). Therefore, the objective of this study is to analyze in vivo patellar mechanism forces and kinematics throughout flexion to determine influencing factors that may lead to patient dissatisfaction.
Methods
Fifty subjects were evaluated in this study, 40 having a Journey II bi-cruciate stabilized (BCS) TKA and 10 having normal, healthy knees. Similar demographics were controlled for each group. Each subject performed a deep knee bend. Kinematics were evaluated using a validated 3D-to-2D fluoroscopic technique while forces were determined using a validated inverse mathematical knee model. A two-tailed t-test was used to evaluate statistical significance.
Introduction
An understanding of anatomic variability can help guide the surgeon on intervention strategies. Well-functioning thumb metacarpophalangeal joints (MCPJ) are essential for carrying out typical daily activities. However, current options for arthroplasty are limited. This is further hindered by the lack of a precise understanding of the geometric variation present in the population. In this paper, we offer new insight into the major modes of geometric variation in the thumb MCP using Statistical Shape Modelling.
Methods
Ten participants free from hand or wrist disease or injury were recruited for CT imaging (Ethics Ref:14/LO/1059)1. Participants were sex matched with mean age 31yrs (range 27–37yrs). Metacarpal (MC1) and proximal phalanx (PP1) bone surfaces were identified in the CT volumes using a greyscale threshold, and meshed. The ten MC1 and ten PP1 segmented bones were aligned by estimating their principal axes using Principal Component Analysis (PCA), and registration was performed to enable statistical comparison of the position of each mesh vertex. PCA was then used again, to reduce the dimensionality of the data by identifying the main ‘modes’ of independent size and shape variation (principal components, PCs) present in the population. Once the PCs were identified, the variation described by each PC was explored by inspecting the shape change at two standard deviations either side of the mean bone shape.
Introduction
Excessive standing posterior pelvic tilt (PT), lumbar spine stiffness, low pelvic Incidence (PI), and severe sagittal spinal deformity (SSD) have been linked to increased dislocation rates. We aimed to compare the prevalence of these 4 parameters in unstable and stable primary Total Hip Arthroplasty (THA) patients.
Methods
In this retrospective cohort study, 40 patients with instability following primary THA for osteoarthritis were referred for functional analysis. All patients received lateral X-rays in standing and flexed seated positions to assess functional pelvic tilt and lumbar lordosis (LL). Computed tomography scans were used to measure pelvic incidence and acetabular cup orientation. Literature thresholds for “at risk” spinopelvic parameters were standing pelvic tilt ≤ −10°, lumbar flexion (LLstand – LLseated) ≤ 20°, PI ≤ 41°, and sagittal spinal deformity (PI – LLstand mismatch) ≥ 10°. The prevalence of each risk factor in the dislocation cohort was calculated and compared to a previously published cohort of 4042 stable THA patients.
Introduction
Varus alignment in total knee replacement (TKR) results in a larger portion of the joint load carried by the medial compartment.[1] Increased burden on the medial compartment could negatively impact the implant fixation, especially for cementless TKR that requires bone ingrowth. Our aim was to quantify the effect varus alignment on the bone-implant interaction of cementless tibial baseplates. To this end, we evaluated the bone-implant micromotion and the amount of bone at risk of failure.[2,3]
Methods
Finite element models (Fig.1) were developed from pre-operative CT scans of the tibiae of 11 female patients with osteoarthritis (age: 58–77 years). We sought to compare two loading conditions from Smith et al.;[1] these corresponded to a mechanically aligned knee and a knee with 4° of varus. Consequently, we virtually implanted each model with a two-peg cementless baseplate following two tibial alignment strategies: mechanical alignment (i.e., perpendicular to the tibial mechanical axis) and 2° tibial varus alignment (the femoral resection accounts for additional 2° varus). The baseplate was modeled as solid titanium (E=114.3 GPa; v=0.33). The pegs and a 1.2 mm layer on the bone-contact surface were modeled as 3D-printed porous titanium (E=1.1 GPa; v=0.3). Bone material properties were non-homogeneous, determined from the CT scans using relationships specific to the proximal tibia.[2,4] The bone-implant interface was modelled as frictional with friction coefficients for solid and porous titanium of 0.6 and 1.1, respectively. Introduction
Sagittal pelvic tilt (SPT) can change with spinal pathologies and fusion. Change in the SPT can result in impingement and hip instability. Our aim was to determine the magnitude of the SPT change for hip instability to test the hypothesis that the magnitude of SPT change for hip instability is less than 10° and it is not similar for different hip motions.
Methods
Hip implant motions were simulated in standing, sitting, sit-to-stand, bending forward, squatting and pivoting in Matlab software. When prosthetic head and liner are parallel, femoral head dome (FHD) faces the center of the liner. FHD moves toward the edge of the liner with hip motions. The maximum distance between the FHD and the center in each motion was calculated and analyzed. To make the results more reliable and to consider the possibility of bony impingement, when the FHD approached 90% of the distance between the liner-center and liner-edge, we considered the hip “in danger for dislocation”. The implant orientations and SPT were modified by 1-degree increments and we used linear regression with receiver operating characteristic (ROC) curve and area under the curve (AUC) to determine the magnitude of SPT change that could cause instability.
Introduction
The fixation of press-fit orthopaedic devices depends on the mechanical properties of the bone that is in contact with the implants. During the press-fit implantation, bone is compacted and permanently deformed, finally resulting in the mechanical interlock between implant and bone. For the development and design of new devices, it is imperative to understand these non-linear interactions. One way to investigate primary fixation is by using computational models based on Finite Element (FE) analysis. However, for a successful simulation, a proper material model is necessary that accurately captures the non-linear response of the bone. In the current study, we combined experimental testing with FE modeling to establish a Crushable Foam model (CFM) to represent the non-linear bone biomechanics that influences implant fixation.
Methods
Mechanical testing of human tibial trabecular bone was done under uniaxial and confined compression configurations. We examined 62 human trabecular bone samples taken from 8 different cadaveric tibiae to obtain all the required parameters defining the CFM, dependent on local bone mineral density (BMD). The derived constitutive rule was subsequently applied using an in-house subroutine to the FE models of the bone specimens, to compare the model predictions against the experimental results.
Background
Artificial total knee designs have revolutionized over time, yet 20% of the population still report dissatisfaction. The standard implants fail to replicate native knee kinematic functionality due to mismatch of condylar surfaces and non-anatomically placed implantation. (Daggett et al 2016; Saigo et al 2017). It is essential that the implant surface matches the native knee to prevent Instability and soft tissue impingement. Our goal is to use computational modeling to determine the ideal shapes and orientations of anatomically-shaped components and test the accuracy of fit of component surfaces.
Methods
One hundred MRI scans of knees with early osteoarthritis were obtained from the NIH Osteoarthritis Initiative, converted into 3D meshes, and aligned via an anatomic coordinate system algorithm. Geomagic Design X software was used to determine the average anterior-posterior (AP) length. Each knee was then scaled in three dimensions to match the average AP length. Geomagic's least-squares algorithm was used to create an average surface model. This method was validated by generating a statistical shaped model using principal component analysis (PCA) to compare to the least square's method. The averaged knee surface was used to design component system sizing schemes of 1, 3, 5, and 7 (fig 1). A further fifty arthritic knees were modeled to test the accuracy of fit for all component sizing schemes. Standard deviation maps were created using Geomagic to analyze the error of fit of the implant surface compared to the native femur surface.
Objectives
Articular cartilage damage is a primary outcome of pre-clinical and clinical studies evaluating meniscal and cartilage repair or replacement techniques. Recent studies have quantitatively characterized India Ink stained cartilage damage through light reflectance and the application of local or global thresholds. We develop a method for the quantitative characterisation of inked cartilage damage with improved generalisation capability, and compare its performance to the threshold-based baseline approach against gold standard labels.
Methods
The Trainable WEKA Segmentation (TWS) tool (Arganda-Carreras et al., 2017) available in Fiji (Rueden et al., 2017) was used to train two separate Random Forest classifiers to automatically segment cartilage damage on ink stained cadaveric ovine stifle joints. Gold standard labels were manually annotated for the training, validation and test datasets for each of the femoral and tibial classifiers. Each dataset included a sample of medial and lateral femoral condyles and tibial plateaus from various stifle joints, selected to ensure no overlap across datasets according to ovine identifier. Training was performed on the training data with the TWS tool using edge, texture and noise reduction filters selected for their suitability and performance. The two trained classifiers were then applied to the validation data to output damage probability maps, on which a threshold value was calibrated. Model predictions on the unseen test set were evaluated against the gold standard labels using the Dice Similarity Coefficient (DSC) – an overlap-based metric, and compared with results for the baseline global threshold approach applied in Fiji as shown in Figures 1 and 2.
A common location for radius fracture is the proximal radial head. With the arm in neutral position, the fracture usually happens in the anterolateral quadrant (Lacheta et al., 2019).
If traditional surgeries are not enough to induce bone stabilization and vascularization, or the fracture can be defined grade III or grade IV (Mason classification), a radial head prosthesis can be the optimal compromise between bone saving and recovering the “terrible triad”.
A commercially available design of radial head prosthesis such as Antea (Adler Ortho, Milan, Italy) is characterized by flexibility in selecting the best matching size for patients and induced osteointegration thanks to the Ti-Por® radial stem realized by 3D printing with laser technique (Figure 1). As demonstrated, Ti-Por® push-out resistance increased 45% between 8 −12 weeks after implantation, hence confirming the ideal bone-osteointegration. Additional features of Antea are: bipolarity, modularity, TiN coating, radiolucency, hypoallergenic, 10° self-aligning.
The osteointegration is of paramount importance for radius, in fact the literature is unfortunately reporting several clinical cases for which the fracture of the prosthesis happened after bone-resorption. Even if related to an uncommon activity, the combination of mechanical resistance provided by the prosthesis and the stabilization due to the osteointegration should cover also accidental movements.
Based upon Lacheta et al. (2019), after axial compression-load until radii failure, all native specimens survived a compression-load of 500N, while the failure happened for a mean compression force of 2560N.
The aim of this research study was to test the mechanical resistance of a radial head prosthesis obtained by 3D printing. In detail, a finite element analysis (FEA) was used to understand the mechanical resistance of the core of the prosthesis and the potential bone fracture induced in the radius with simulated bone- resorption (Figure 2a). The critical level was estimated at the height for which the thickness of the core is the minimum (Figure 2b).
Considered boundary conditions:
- Full-length prosthesis plus radius out of the cement block equal to 60mm (Figure 2a); - Bone inside the cement equal to 60mm (Figure 2b); - Load inclined 10° epiphysiary component (Figure 2c); - Radius with physiological or osteoporotic bone conditions; - Load (concentrated in the sphere simulating full transmission from the articulation) of 500N or 1300N or 2560N.
Figure 3 shows the results in terms of maximum stress on the core of the prosthesis and the risk of fracture (Schileo et al., 2008).
According to the obtained results, the radial head prosthesis shows promising mechanical resistance despite of the simulated bone-resorption for all applied loads except for 2560N. The estimated mechanical limit for the material in use is 200MPa. The risk of fracture is in agreement with the experimental findings (Lacheta et al. (2019)), in fact bone starts to fail for the minimum reported failure load, but only for osteoporotic conditions.
The presented FEA aimed at investigating the behavior of a femoral head prostheses made by 3D printing with simulated bone-resorption. The prosthesis shows to be a skilled solution even during accidental loads.

Introduction
Total Hip Arthroplasty (THA) devices are now increasingly subjected to a progressively greater range of kinematic and loading regimes from substantially younger and more active patients. In the interest of ensuring adequate THA solutions for all patient groups, THA polyethylene acetabular liner (PE Liner) wear representative of younger, heavier, and more active patients (referred to as HA in this study) warrants further understanding.
Previous studies have investigated HA joint related morbidity [1]. Current or past rugby players are more likely to report osteoarthritis, osteoporosis, and joint replacement than a general population.
This investigation aimed to provide a preliminary understanding of HA patient specific PE liner tribological performance during Standard Walking (SW) gait in comparison to IS0:14242-1:2014 standardized testing.
Materials and Methods
Nine healthy male subjects volunteered for a gait lab-based study to collect kinematics and loading profiles. Owing to limitations in subject selection, five subjects wore a weighted jacket to increase Body Mass Index ≥30 (BMI). An induced increase in Bodyweight was capped (<30%BW) to avoid significantly effecting gait [3] (mean=11%BW).
Six subjects identified as HA per BMI≥30, but with anthropometric ratios indicative of lower body fat as previously detailed by the author [2] (Waist-to-hip circumference ratio and waist circumference-to-height ratio). Three subjects identified as Normal (BMI<25). Instrumented force plate loading profiles were scaled (≈270%BW) in agreement with instrumented hip force data [4].
A previously verified THA (Pinnacle® Marathon® 36×56mm, DePuy Synthes) Finite Element Analysis wear model based on Archard's law and modified time hardening model [5] was used to predict geometrical changes due to wear and deformation, respectively (Figure 1). Subject dependent kinematic and loading conditions were sampled to generate, for both legs, 19 SW simulation runs using a central composite design of response surface method.
Introduction
Current methodologies for designing and validating existing THA systems can be expensive and time-consuming. A validated mathematical model provides an alternative solution with immediate predictions of contact mechanics and an understanding of potential adverse effects. The objective of this study is to demonstrate the value of a validated forward solution mathematical model of the hip that can offer kinematic results similar to fluoroscopy and forces similar to telemetric implants.
Methods
This model is a forward solution dynamic model of the hip that incorporates the muscles at the hip, the hip capsule, and the ability to modify implant position, orientation, and surgical technique. Muscle forces are simulated to drive the motion, and a unique contact detection algorithm allows for virtual implantation of components in any orientation. Patient-specific data was input into the model for a telemetric subject and for a fluoroscopic subject.
Taper corrosion and fretting have been associated with oxide layer abrasion and fluid ingress that contributes to adverse local tissue reactions with potential failure of the hip joint replacement[1,2]. Both mechanisms are considered to be affected by the precise nature of the taper design[3]. Indeed relative motion at the taper interface that causes fretting damage and wear effects, such as pistoning and rocking, have been described following analysis of implants at retrieval[4,5]. However, there is much less reported about the mechanisms that allow the fluid ingress/egress at the taper interface which would drive corrosion. Thus the aim of the present study was to investigate the effect of trunnion design on the gap opening and taper relative motions under different load scenarios and taper designs.
A 3-D finite element model of a 40mm CoCr modular femoral head and a Ti6Al4V trunnion was established in Abaqus CAE/2018. Femoral head and trunnion geometries were meshed with an element (C3D8) size of 0.17mm. Tapers were assembled by simulating a range of impact forces (AF); taper interface behaviour was evaluated under physiological forces and frictional moments simulated during walking activity[6], assuming different coefficients of friction (CF), Figure 1. The output involved the total and normal relative motion of the surfaces at the taper interface.
The model predicted for a taper mismatch of 0.36° which, when combined with an assembly force of 2kN, generated the largest taper gap opening (59.2mm) during walking, Figure 2. In all trunnion designs the largest normal relative motion coincided with heel strike in the gait cycle (0–5%). The taper gap and normal relative motions were related to the initial taper lock area. Furthermore, the direction of the total motion was different in all three taper mismatches, with a shift in the direction towards the normal of the surface as the taper mismatch increased, Figure 3. By contrast, the direction of the normal relative motions did not change with different trunnion designs. Contact patterns were asymmetrical and contact areas varied throughout the walking activity; contact pressure and the largest taper gap were located on the same side of the taper, suggesting toggling of the trunnion.
The relationship between taper gap opening and initial taper lock contact area suggests that the taper contact area functions as a fulcrum in a lever mechanism. Large taper mismatches create larger relative motions that will not only create more wear and fretting damage but also larger normal relative motions. This may allow fluid ingress into the taper interface and/or the egress of fluid along with any metal wear particles into the body. This increased understanding of the taper motion will result in improved designs and ultimately taper performance.

The vast majority of total hip replacements (THR) implanted today enable modularity by means of a tapered junction; based on the Morse taper design introduced for cutting tools in the 19th Century 1. Morse-type tapers at the head-stem junction provide many benefits, key for a successful surgical outcome such as wider component selection and restoration of better biomechanics 2. However, moving from mono-block to modular designs has not been without its issues. Fluid ingress and motion at the interface has led to a complex multifactorial degradation mechanism better known as fretting-corrosion 3. Fretting-corrosion products created at the junction are commonly associated with adverse local tissue reactions 4.
There is a wide variation in the taper junction of THR differing quite significantly from Morse's original design. Performance of the taper junction has been found to vary with different designs 5,6. However, there is still a lack of common understanding of what design inputs makes a ‘good’ modular taper interface. The aim of this study was to better understand the links between implant design and fretting-corrosion initially focussing on the role of angular mismatch between male and female taper. A combination of experimental approaches with the aid of computational models to assist understanding has been adopted. A more descriptive understanding between taper design, engagement, motion and fretting-corrosion will be developed.
Three different sample designs were created to represent the maximum range of possible angular mismatches seen in clinically available THR modular tapers (Matched: 0.020 ±0.002 °, Proximal: 0.127 ±0.016 °, Distal: −0.090 ±0.002 °). Head-stem components were assembled at 2 kN. Motion and fretting-corrosion at the interface was simulated under incremental uniaxial sinusoidal loading between 0.5–4 kN at 8 intervals of 600 cycles. The different types of motions at the interface was measured using a developed inductance circuit composed of four sensing coils, digital inductance converter chip (LDC1614, Texas Instruments, US) and microcontroller (myRIO, National Instruments, US). Fretting-corrosion was measured using potentiostatic electrochemical techniques with an over potential of +100 mV vs OCP (Ivium, NL). Complimentary finite element (FE) models were created in Ansys (Ansys 19.2, US).
Under uniaxial loading, the ‘matched’ modular taper assemblies corroded most and allowed the greatest pistoning motion due to a seating action. ‘Distal’ and ‘proximal’ engaged modular tapers showed reduced corrosion and seating when compare to the ‘matched’ components. However the kinetics of corrosion and motion were interface dependent. It is hypothesized, and complimented by FEA analysis, that lower initial contact stress in the ‘matched’ modular tapers allows for greater subsidence and depassivation of the oxide layer and higher corrosion. ‘Matched’ modular tapers allowed less rotational and toggling motions compared to mismatched tapers, suggesting a reduced mismatch might perform better once the heads have seated over time. Future work involves tests conducted under a surgically relevant impaction force and physiological loading kinematics to develop this descriptive link between taper design, engagement and performance.
Introduction
Dislocation is a major cause of Total Hip Arthroplasty (THA) early failure and is highly influenced by surgical approach and component positioning. Robotic assisted arthroplasty has been developed to improve component positioning and therefore reduce post-operative complications.
The purpose of this study was to assess dislocation rate in robotic total hip arthroplasty performed with three different surgical approaches.
Methods
All patients undergoing Robotic Arm-Assisted THA at three centers between 2014 and 2019 were included for assessment. After exclusion, 1059 patients were considered; an anterior approach was performed in 323 patients (Center 2), lateral approach in 394 patients (Center 1 and Center 2) and posterior approach in 394 patients (Center 1 and Center 3).
Episodes of THA dislocation at 6 months of follow up were recorded. Stem anteversion, Cup anteversion, Cup inclination and Combined Anteversion were collected with the use of the integrated navigation system.
Cumulative incidence (CI), incidence rate (IR) and risk ratio (RR) were calculated with a confidence interval of 95%.

Many groups consider passive flexion to be a good indicator of postoperative success, to the point where this outcome directly influences certain outcome scores such as Knee Society Scores (KSS). However, it is alternatively believed that normal-like kinematics result in better TKA outcomes, and previous fluoroscopy studies have demonstrated that there are many parameters that affect weight-bearing range-of-motion. The objective of this study to investigate the correlations between patient-reported outcomes, passive flexion, and weight-bearing knee kinematics.
Methods
The femorotibial kinematics, passive and weight-bearing range-of-motion, and KOOS and KSS for 291 TKA subjects were collected in a retrospective study. The average age, BMI, and post-op time was 69.2±7.2 years, 29.3±4.6, and 22.4±16.3 months, respectively. Pearson correlation analysis was used to find the statistical correlations between the various parameters, and two-tailed t-tests were carried out to find statistical differences.
Introduction
Achieving a balanced joint with neutral alignment is not always possible in total knee arthroplasty (TKA). Intra-operative compromises such as accepting some joint imbalance, non-neutral alignment or soft-tissue release may result in worse patient outcomes, however, it is unclear which compromise will most impact outcome. In this study we investigate the impact of post-operative soft tissue balance and component alignment on postoperative pain.
Methods
135 patients were prospectively enrolled in robot assisted TKA with a digital joint tensioning tool (OMNIBotics with BalanceBot, Corin USA) (57% female; 67.0 ± 8.1 y/o; BMI: 31.9 ± 4.8 kg/m2). All surgeries were performed with a PCL sacrificing tibia or femur first techniques technique, using CR femoral components and a deep dish tibial insert (APEX, Corin USA). Gap measurements were acquired under load (average 80 N) throughout the range of motion during trialing with the tensioning tool inserted in place of the tibial trial. Component alignment parameters and post-operative joint gaps throughout flexion were recorded. Patients completed 1-year KOOS pain questionnaires. Spearman correlations and Mann-Whitney-U tests were used to investigate continuous and categorical data respectively. All analysis performed in R 3.5.3.
Introduction
A bicruciate retaining (BCR) TKA is thought to maintain a closer resemblance to the native knee kinematics compared to a posterior cruciate retaining (CR) TKA. With BCR TKAs retainment of the anterior cruciate ligament (ACL) facilitates proprioception and balance which is thought to lead to more natural knee kinematics and increased functional outcome. The aim of this study was to quantify and compare the kinematics of a BCR and CR TKA during functional tests.
Materials and Methods
In this patient-blinded randomized controlled trial, a total of 40 patients with knee osteoarthritis were included, 18 of them received a BCR TKA (Vanguard XP, Zimmer-Biomet) and 22 received a CR TKA (Vanguard CR, Zimmer-Biomet). Fluoroscopic analysis was done 1 year post-operatively. The main outcome was posterior femoral rollback (i.e. translation of the femorotibial contact point (CP)) of the BCR and CR TKA during a step-up test. Secondary, the kinematics during a lunge test were quantified as anterior-posterior (AP) translation of the femorotibial CP. Independent student t-tests (or non-parametric equivalent) were used to analyze the effect of BCR versus CR TKA on these measures, to correct for the multiple testing problem post-hoc Bonferroni-Holm corrections were applied.
Background
In the United Kingdom, over 1 million elective surgeries were cancelled due to COVID-19, resulting in over 1.9 million people now waiting more than 4 months for their procedure – 3x the number last year. To address this backlog, the healthcare service has been asked to develop locally-designed ‘COVID-light’ facilities. In our local system, 822 patients awaited orthopaedic surgery when elective surgery was permitted to resume. The phased return of service required a careful and pragmatic prioritisation of patients, to protect resources, patients, and healthcare workers.
Aims
We aim to describe how the COVID-19 Algorithm for Resuming Elective Surgery (CARES) was used to consider 1) Which type of operation and patient should be prioritised? and 2) Which patients are safe to undergo surgery? The central tenets to this were patient safety, predicted efficacy of the surgery, and delivering compassionate care by considering biopsychosocial factors.
Introduction
Innovations in orthopaedic technology and infrastructure growth often require significant funding. Although an increasing trend has been observed for third-party investments into medical startups and physician practices, no study has examined the role of this funding in orthopaedics, including the influence of venture capital (VC). Therefore, this study analyzed trends in VC investments related to the field of orthopaedic surgery, as well as the characteristics of companies receiving said investments.
Methods
Venture capital investments into orthopaedic-related businesses were reviewed from 2000–2019 using Capital IQ, a proprietary market intelligence platform documenting financial transactions. The dataset was initially filtered to include healthcare-related venture capital transactions pertaining to the field of orthopaedic surgery. The final list of VC investments and their corresponding businesses were categorized by transaction year, amount (in USD), and orthopaedic subspecialty. The number and sum of VC investments was calculated both annually and cumulatively across the entire study period. Linear regression was used for trend analysis within two distinct, decade-long timeframes (2000–2009 and 2010–2019) and one-way analysis of variance was used to assess differences across orthopaedic subspecialties.

There is debate regarding whether the use of computer-assisted technology, such as navigation and robotics, has any benefit on clinical or patient reported outcomes following total knee arthroplasty (TKA). This study aims to report on the association between intraoperative use of technology and outcomes in patients who underwent primary TKA.

We retrospectively reviewed 7,096 patients who underwent primary TKA from 2016–2020. Patients were stratified depending on the technology utilized intraoperatively: navigation, robotics, or no technology. The iASSIST system is a portable, accelerometer base with electronic navigation used for total knee arthroplasty (TKA) which guides the surgeon to align and validate bone resection during the surgical procedure. The purpose of this study was to compare the radiological outcome between accelerometer base iASSIST system and the conventional system.
Method
A prospective study between two group of 36 patients (50 TKA) of primary osteoarthritis of the knee who underwent TKA using iASSIST ™ or conventional method (25 TKA in each group) from January 2018 to December 2019. A single surgeon performs all operations with the same instrumentation and same surgical approach. Pre-operative and postoperative management protocol are same for both groups. All patients had standardized scanogram (full leg radiogram) performed post operatively to determine mechanical axis of lower limb, femoral and tibial component alignment.
Result
There was no significant difference between the 2 groups for Age, Gender, Body mass index, Laterality and Preoperative mechanical axis(p>0.05). There was no difference in proportion of outliers for mechanical axis (p=0.91), Coronal femoral component alignment angle (p=0.08), Coronal tibial component alignment angle (p=1.0). The mean duration of surgery, postoperative drop in Hb, number of blood transfusion didn't show significant difference between 2 groups (p>0.05).

Primary robotic-arm assisted total hip arthroplasty (THA) yields more accurate and reproducible acetabular cup placement, nonetheless, data is scarce in terms of outcomes. The purpose of the present study was to report on patient-reported outcomes (PROMs) in a large group of patients who underwent robotic-arm assisted THA. The authors hypothesized that (1) patients who underwent robotic-arm assisted primary THA would achieve favorable and significant improvement in PROMs, (2) an accurate and reproducible acetabular cup placement with respect to the defined SafeZones would be obtained, and (3) a low rate of THA dislocation would be observed.
Methods
Prospectively collected data were retrospectively reviewed between April 2012 to May 2017. Primary THA using Mako Robotic-Arm [Mako Surgical Corp. (Stryker), Fort Lauderdale, FL, USA] with minimum two-year follow-up for the Harris Hip Score (HHS) and the Forgotten Joint Score-12 (FJS-12) were included. Exclusion criteria were: bodymass index (BMI) > 40 kg/m2, age < 21-year old, worker's compensation, or unwilling to participate. Visual analog scale (VAS) for pain and patient satisfaction were obtained. Intraoperative measurements for leg-length, global offset, acetabular inclination and version were documented.
Background
Total hip arthroplasty (THA) has been and continues to be the gold standard for treatment of end-stage osteoarthritis. With each year, implant characteristics are evolving to increase patient-reported outcomes and decrease complications.
Methods
Data were prospectively collected on patients who underwent THA with Corin-type implants (both cup and stem) and THA using Stryker implants between June 2011 and July 2016. A 1:1 propensity match was performed using the following 5 covariates: age, body mass index, gender, Charlson score and smoking status. Surgical outcomes were assessed at minimum 2-year follow-up using the Forgotten Joint Score (FJS), Harris Hip Score (HHS), Veterans RAND 12-item physical and mental health survey, Short Form 12 physical and mental health survey, Visual Analog Score (VAS), and patient satisfaction. The exclusion criteria were previous hip condition/surgery, workers compensation, or were unwilling.

Robotic-arm assisted knee arthroplasty (rKA) has been associated with improved clinical, radiographic, and patient-reported outcomes. There is a paucity of literature, however, addressing its cost effectiveness. In the context of an integrated health system with an insurance plan and single source comprehensive data warehouse for electronic health records and claims data, we present an evaluation of healthcare costs and utilization associated with manual knee arthroplasty (mKA) versus rKA. We also examine the influence of rKA technology on surgeons’ practice patterns.
Methods
Practice patterns of KA were assessed 18 months before and after introduction of robotic technology in April 2018. For patients also insured through the system's health plan, inpatient costs (actual costs recorded by health system), 90-day postoperative costs (allowed amounts paid by insurance plan), and 90-day postoperative utilization (length of stay, home health care visits, rehabilitation visits) were compared between mKA and rKA patients, stratified by total (TKA) or unicompartmental (UKA) surgery. Linear regression modeling was used to compare outcomes between the two pairs of groups (mKA vs. rKA, for both UKA and TKA). Log-link function and gamma error distribution was used for costs. All analyses were done using SAS statistical software, with p<0.05 considered statistically significant.

Total knee and hip arthroplasty were the main arthroplasty surgery performed in Brazil. In Brazil, around 50% of arthroplasty surgeries are performed by the public health system, knowing as SUS 1. SUS is the biggest public and universal health system in the world, attending 100% of the Brazilian population (220 million), with 2020's expected budget of US$ 31 billion 2,3.

In March 2020, due to the COVID-19 pandemics, the Brazilian Health Ministry oriented for the postponing of elective surgery, to prioritize combating the virus 5. This postponing, it was important but will cause an impact in the economy at the public health system from Brazil this year and in the next years. Although surgical remedies tend to be the long-term solutions for patients with osteoarthritis (OA), many alternatives exist that offer the potential to slow progression, alleviate pain, and/or restore function. One such option is the unloader OA knee brace. The objective of this study was to assess the in vivo medial joint space narrowing with and without the brace during weight-bearing portion of gait.
Methods
Twenty subjects were evaluated after being clinically assessed by a single surgeon to be bone-on-bone on the medial side. In vivo gait kinematics were collected using a validated 3D-to-2D fluoroscopic registration technique (Figure 1). Subjects were asked to first walk on a treadmill without a brace (Figure 2), and then, after a qualified technician fit a properly sized brace to each subject, they were asked to walk again (Figure 3). In vivo fluoroscopic images were captured and registered at heel-strike (HS) and mid stance (MS) for both scenarios. CT scans were used to acquire the patient-specific bone models that were used in the registration process.
Background
Conventional instrumented total knee arthroplasty uses fixed angles for bony cuts followed by soft tissue releases to achieve balance. Robotic-assisted surgery allows for soft tissue balancing first then bony resection. The changes to the implant position from conventional instrumented surgery were measured and recorded.

A single center, retrospective study reviewed consecutive total knee replacement surgeries over a 12 month period utilizing robotic pre-planning and balancing techniques. Changes to femoral and tibial varus/valgus and femoral rotation from traditional instrumented surgery positions were analyzed.
Introduction
Wearable sensors are promising tools for fast clinical gait evaluations in individuals with osteoarthritis (OA) of the knee and hip. However, gait assessments with wearable sensor are often limited to relatively simple straight-ahead walking paradigms. Parameters reflecting more complex and relevant aspects of gait, including dual-tasking, turning, and compensatory upper body motion are often overlooked in literature. The aim of this study was to investigate turning, dual-task performance, and upper body motion in individuals with knee or hip OA in addition to spatiotemporal gait parameters, taking shared covariance between gait parameters into account.
Methods
Gait was compared between individuals with unilateral knee (n=25) or hip (n=26) OA scheduled for joint replacement, and healthy controls (n=27). For 2 minutes, subjects walked back-and-forth a 6 meter trajectory making 180 degree turns, with and without a secondary cognitive task. Gait parameters were collected using four inertial measurement units on feet, waist, and trunk. To test if turning, dual-tasking, and upper body motion had added value above common spatiotemporal parameters, a factor analysis was conducted. Standardized mean differences were computed for the comparison between knee or hip OA and healthy controls. One gait parameter was selected per gait domain based on factor loading and effect size for the comparison between OA groups and healthy controls.
Introduction & Aims
In other medical fields, smart implantable devices are enabling decentralised monitoring of patients and early detection of disease. Despite research-focused smart orthopaedic implants dating back to the 1980s, such implants have not been adopted into regular clinical practice. The hardware footprint and commercial cost of components for sensing, powering, processing, and communicating are too large for mass-market use. However, a low-cost, minimal-modification solution that could detect loosening and infection would have considerable benefits for both patients and healthcare providers. This proof-of-concept study aimed to determine if loosening/infection data could be monitored with only two components inside an implant: a single-element sensor and simple communication element.
Methods
The sensor and coil were embedded onto a representative cemented total knee replacement. The implant was then cemented onto synthetic bone using polymethylmethacrylate (PMMA). Wireless measurements for loosening and infection were then made across different thicknesses of porcine tissue to characterise the sensor's accuracy for a range of implantation depths. Loosening was simulated by taking measurements before and after compromising the implant-cement interface, with fluid influx simulated with phosphate-buffered saline solution. Elevated temperature was used as a proxy for infection, with the sensor calibrated wirelessly through 5 mm of porcine tissue across a temperature range of 26–40°C.
Introduction
The ability to create patient-specific implants (PSI) at the point-of-care has become a desire for clinicians wanting to provide affordable and customized treatment. While some hospitals have already adopted extrusion-based 3D printing (fused filament fabrication; FFF) for creating non-implantable instruments, recent innovations have allowed for the printing of high-temperature implantable polymers including polyetheretherketone (PEEK). With interest in FFF PEEK implants growing, it is important to identify methods for printing favorable implant characteristics such as porosity for osseointegration.
In this study, we assess the effect of porous geometry on the cell response and mechanical properties for FFF-printed porous PEEK. We also demonstrate the ability to design and print customized porous implants, specifically for a sheep tibial segmental defect model, based on CT images and using the geometry of triply periodic minimal surfaces (TPMS).
Methods
Three porous constructs – a rectilinear pattern and gyroid/diamond TPMSs – were designed to mimic trabecular bone morphology and manufactured via PEEK FFF. TPMSs were designed by altering their respective equation approximations to achieve desired porous characteristics, and the meshes were solidified and shaped using a CAD workflow. Printed samples were mCT scanned to determine the resulting pore size and porosity, then seeded with pre-osteoblast cells for 7 and 14 days. Cell proliferation and alkaline phosphatase activity (ALP) were evaluated, and the samples were imaged via SEM. The structures were tested in compression, and stiffness and yield strength values were determined from resulting stress-strain plots. Roughness was determined using optical profilometry. Finally, our process of porous structure design/creation was modified to establish a proof-of-concept workflow for creating PSIs using geometry established from segmented sheep tibia CT images.
Introduction
According to American Joint Replacement Registry, particle mediated osteolysis represents 13 % of the knee revision surgeries performed in the United States. The comprehension of mechanical and wear properties of materials envisioned for TJR is a key step in product development. Furthermore, the maintenance of UHMWPE mechanical properties after material modification is an important aspect of material success. Initial studies conducted by our research group demonstrated that the incorporation of ibuprofen in UHMWPE had a minor impact on UHMWPE physicochemical and mechanical properties. Drug release was also evaluated and resulted in an interesting profile as a material to be used as an anti-inflammatory system. Therefore, the present study investigated the effect of drug release on the mechanical and biological properties of ibuprofen-loaded UHMWPE.
Experimental
UHMWPE resin GUR 1020 from Ticona was for sample preparation. Samples with drug concentrations of 3% and 5% wt were consolidated as well as samples without anti-inflammatory addition through compression molding at 150 °C and 5 MPa for 15 minutes. Mechanical properties were evaluated via the tensile strength experiment (ASTM D638) and dynamic mechanic tests. Wear resistance was measured using the pin on disc (POD) apparatus. Finally, cytotoxicity analysis was conducted based on ISO 10993–5.
Introduction
Innovations in surgical robotics and navigation have significantly improved implant placement accuracy in total knee arthroplasty (TKA). However, many comparative studies have not been shown to substantially improve revision rates or other clinical outcome scores. We conducted a simulation study based on the reported distribution of patient-specific characteristics and estimated potential effect of coronal plane alignment (CPA) on risk of revision to evaluate the hypothesis that most published study designs in this area have been too underpowered to detect improvements in revision rates.

To model previously reported studies, we generated a series of simulated TKA patient populations, assigning each patient a set of patient-specific factors (age at index surgery, BMI, and sex (Fig.1a)), as well as one surgeon-controlled factor (CPA) (Fig.1b) based on registry data and published literature. To evaluate power associated with a study design, we ran a Monte Carlo simulation generating 10,000 simulated populations of ten different cohort sizes. We divided the patient population into two groups: one group was assigned CPAs governed by the precision of a navigated/robotic approach (σ=1.5°), and the other CPAs governed by the precision of a conventional approach (σ=3°). We then simulated the time to failure for each patient, computed the corresponding Kaplan-Meier survival curves, and applied a Log-Rank test to each study to test for statistical difference. From the 10,000 simulations associated with each cohort size, we determined the percentage of simulated studies that found a statistically significant difference at each time point.
Introduction
Surface sensor technology provides useful information about the status of an individual's health and been available for many years, but has not been widely adopted by orthopaedic surgeons. However, its usage may be become more prominent as COVID-19 has created a shift towards telemedicine. This study reports the use of a joint specific surface sensor to remotely monitor the recovery of patients who underwent knee replacement surgery prior to the enforced stay-at-home social distancing necessitated by the COVID-19 pandemic.
Methods
The study group consisted of 29 patients who underwent primary, unilateral total knee arthroplasty (TKA). A knee joint specific surface sensor (TracPatch™) was placed following surgery and kept on patients for 3 weeks postoperatively. The patients’ range of motion (ROM), exercise compliance, distance walked, pain, skin temperature, and incision appearance were monitored and transmitted electronically to health care providers. Patients were grouped by gender, age and BMI for analysis of functional outcome measurements.
Ti-6Al-4V is the most common alloy used for orthopaedic implants. Its popularity is due to low density, superior corrosion resistance, good osseointegration and lower elastic modulus when compared to other commonly used alloys such as CoCrMo and stainless steel. In fact, the use of Ti64 has even further increased lately since recent controversy around adverse local tissue reactions and implant failure related to taper corrosion of CoCrMo alloy. However, implants made from Ti64 can fail in some cases due to fatigue fracture, sometimes related to oxide induced stress corrosion cracking or hydrogen embrittlement, or preferential corrosion of the beta phase. Studies performed with Ti-6Al-4V do often not consider that the alloy itself may have a range of characteristics that can vary and could significantly impact the implant properties. These variations are related to the material microstructure which depends not only on chemical composition, but also the manufacturing process and subsequent heat treatments. Different microstructures can occur in implants made form wrought alloys, cast alloys, and more recently, additive manufactured (AM) alloys. Implant alloy microstructure drives mechanical and electrochemical properties. Therefore, this study aims to analyse the microstructure of Ti-6Al-4V alloy of additive manufactured and conventional retrieved orthopaedic implants such as acetabular cups, tibial trays, femoral stem and modular neck by means of electron backscatter diffraction (EBSD). Microstructural features of interest include grains shape and size, phase content and distribution, preferred grain orientation (texture), alloying elements distribution (homogenization) and presence of impurities. Additionally, we demonstrate the direct impact of different microstructural features on hardness. We analysed 17 conventional devices from 6 different manufacturers, 3 additive manufactured devices from 2 different manufactures and 1 control alloy (bar stock). The preliminary results showed that even though all implants have the same chemical composition, their microstructural characteristics vary broadly. Ti64 microstructure of conventional alloys could be categorized in 3 groups: equiaxed grains alloys (Fine and Coarse), bimodal alloys and dendritic alloys. The additive manufactured implants were classified in an additional group on its own which consists of a needle-like microstructures - similar to Widmanstätten patterns, Fig. 1, with a network of β phase along α phase grains. Furthermore, AM alloys exhibited residual grain boundaries from the original β grains from the early stage of the solidification process, Fig. 2. These characteristics may have implication on the fatigue and corrosion behaviour. In addition, it we observed inhomogeneous alloying element distribution in some cases, Fig. 3, especially for the additive manufactured alloys, which also may have consequences on corrosion behaviour. Finally, the hardness testing revealed that the implants with large grain size, such as AM alloys, exhibit low hardness values, as expected, but also the amount of beta phase correlated positively with lower hardness. Grain aspect ratio and beta phase grain size correlated positively with higher hardness. In summary, we found that common Ti64 implants can exhibit a broad variety of different alloy microstructures and the advent of AM alloys introduces an entirely new category. It is imperative to determine the ideal microstructure for specific applications.

Orthopedic implants are subject to wear and release ultra-high molecular weight polyethylene (UHMWPE) debris. Analysis of UHMWPE wear particles is critical in determining the safety and effectiveness of novel orthopedic implants. Complete digestion of periprosthetic tissue and wear fluid is necessary to ensure accurate morphological and quantitative particle analysis. 1a) UHMWPE (GUR 150) particles were generated by Mode I knee wear testing for 1 million cycles in bovine serum. Serum was digested in one of four solutions: 12.2M HCl, 15.8M HNO3, a 1:1 volume ratio of HNO3:HCl (aqua regia), or filtered H2O (control). The serum:solution volume ratio was 1:5 [Niedzwiecki+ 2001, ISO 17853:2011]. Digestion occurred for 60min on a stir plate at 60°C. Each digest was combined with MeOH at a 1:5 digest:MeOH volume ratio and filtered using a 100 nm polycarbonate membrane. The particle-containing membranes were imaged (12 images/membrane) using scanning electron microscopy (SEM) to determine particle characteristics, including quantity, equivalent circular diameter (ECD) and aspect ratio (AR).
Introduction
Pin-tract infections are a common problem in orthopaedic surgery, which limits the time an external fixator or Taylor spatial frame can be applied to a patient. The purpose of our study is to evaluate the ability of a novel implant surface coating — cationic steroid antibiotic (CSA)-44 — to delay or prevent the onset of these infections. This coating mimics endogenous antimicrobial peptides of the innate immune system and has been shown to effectively eradicate biofilms as well as prevent infection and stimulate healing of open, contaminated fractures.
Methods
Surgeries were performed on 20 animals (outbred; Sprague-Dawley strain rats). Each animal received both CSA-coated and standard-of-care titanium pins, with pins randomized to the fifth or sixth vertebrae prior to surgeries. Animals were also randomized to either “Imaging” (imaging analysis) or “Infection” (microbiological analysis) cohorts. Surgeons were blinded to pin types and analyses cohorts. Digital images of pin sites were collected weekly over 12 weeks, and then graded by two orthopaedic surgery residents according to an established Likert scale. Graders were blinded to animal numbers, pin types, and timepoints (Figure 1). For the infection analysis cohort, four specimens per site were subjected to microbiological analysis from each site (i.e. pin, superficial skin swab, deep skin swab, sonicated bone). Background
It is technically challenging to restore hip rotation center exactly in total hip arthroplasty (THA) for patients with end-stage osteoarthritis secondary to developmental dysplasia of the hip (DDH) due to the complicated acetabular morphology changes. In this study, we developed a new method to restore hip rotation center exactly and rapidly in THA with the assistance of three dimensional (3-D) printing technology.
Methods
Seventeen patients (21 hips) with end-stage osteoarthritis secondary to DDH who underwent THA were included in this study. Simulated operations were performed on 3-D printed hip models for preoperative planning. The Harris fossa and acetabular notches were recognized and restored to locate acetabular center. The agreement on the size of acetabular cup and bone defect between simulated operations and actual operations were analyzed. Clinical and radiographic outcomes were recorded and evaluated.
Background
Revision total knee arthroplasty (rTKA) is a high stakes procedure with complex equipment and multiple steps. For rTKA using the ATTUNE system revising femoral and tibial components with sleeves and stems, there are over 240 pieces of equipment that require correct assembly at the appropriate time. Due to changing teams, work rotas, and the infrequency of rTKR, scrub nurses may encounter these operations infrequently and often rely heavily on company representatives to guide them. In turn, this delays and interrupts surgical efficiency and can result in error. This study investigates the impact of a fully immersive virtual reality (VR) curriculum on training scrub nurses in technical skills and knowledge of performing a complex rTKA, to improve efficiency and reduce error.
Method
Ten orthopaedic scrub nurses were recruited and trained in four VR sessions over a 4-week period. Each VR session involved a guided mode, where participants were taught the steps of rTKA surgery by the simulator in a simulated operating theatre. The latter 3 sessions involved a guided mode followed by an unguided VR assessment. Outcome measures in the unguided assessment were related to procedural sequence, duration of surgery and efficiency of movement. Transfer of skills was assessed during a pre-training and post-training assessment, where participants completed multi-step instrument selection and assembly using the real equipment. A pre and post-training questionnaire assessed the participants knowledge, confidence and anxiety.
Introduction
Lumbar spine fusion in patients undergoing THA (total hip arthroplasty) is a known risk factor for hip dislocation with some studies showing a 400% increased incidence compared to the overall THA population. Reduced spine flexibility can effectively narrow the cup anteversion safe zone while alterations in pelvic tilt can alter the center of the anteversion safe zone. The use of precision cup alignment technology combined with patient-specific cup alignment goals based on preoperative assessment has been suggested as a method of addressing this problem. The current study assess the dislocation rate of THA patients with stiff or fused lumbar spines treated using surgical navigation with patient-specific cup orientation goals.
Methods
Seventy-five THA were performed in 54 patients with a diagnosis of lumbar fusion, lumbar disc replacement, and scoliosis with Cobb angles greater than 40 degrees were treated by the senior author (SM) as part of a prospective, non-randomized study of surgical navigation in total hip arthroplasty. All patients were treated using a smart mechanical navigation tool for cup alignment (HipXpert System, Surgical Planning Associates, Inc., Boston, MA). Cup orientation goals were set on a patient-specific basis using supine pelvic tilt as measured using CT. Patients with increased pelvic tilt had a goal for increased cup anteversion and patients with decreased pelvic tilt had a goal for decreased cup anteversion (relative to the anterior pelvic plane coordinate system). Each patient's more recent outpatient records were assessed for history of dislocation, instability, mechanical symptoms, decreased range of motion or progressive pain. Additionally, last clinic radiographs were reviewed to confirm lumbar pathology in the form of spinal surgical hardware.
Introduction
The efficacy of Virtual Reality (VR) as a teaching augment for arthroplasty has not been well examined for unfamiliar multistep procedures such as unicompartmental knee arthroplasty (UKA). This study sought to determine if VR improves surgical competence over traditional procedural preparation when performing a UKA.
Methods
22 Orthopaedic trainees were randomized to training sessions: 1) “VR group” with access to an immersive VR learning module that had been designed in conjunction with the manufacturer or 2) “Guide group” with access to manufacture's technique guide and surgical video. Both groups then performed a full UKA on SawBones models. Surgical competence was assessed via Objective Structures Assessment of Technical Skills (OSATS) validated rating system (max 25 points).
Introduction
Interactions between hip, pelvis and spine, as abnormal spinopelvic movements, have been associated with inferior outcomes following total hip arthroplasty (THA). Changes in pelvis position lead to a mutual change in functional cup orientation, with both pelvic tilt and rotation having a significant effect on version. Hip osteoarthritis (OA) patients have shown reduced hip kinematics which may place increased demands on the pelvis and the spine.
Sagittal and coronal planes assessments are commonly done as these can be adequately studied with anteroposterior and lateral radiographs. However, abnormal pelvis rotation is likely to compromise the outcome as they have a detrimental effect on cup orientation and increased impingement risk. This study aims to determine the association between dynamic motion and radiographic sagittal assessments; and examine the association between axial and sagittal spinal and pelvic kinematics between hip OA patients and healthy controls (CTRL).
Methods
This is a prospective study, IRB approved. Twenty hip OA pre-THA patients (11F/9M, 67±9 years) and six CTRL (3F/3M, 46±18 years) underwent lateral spinopelvic radiographs in standing and seated bend-and-reach (SBR) positions. Pelvic tilt (PT), pelvic-femoral-angle (PFA) and lumbar lordosis (LL) angles were measured in both positions and the differences (Δ) between standing and SBR were calculated. Dynamic SBR and seated maximal-trunk-rotation (STR) were recorded in the biomechanics laboratory using a 10-infrared camera and processed on a motion capture system (Vicon, UK). Direct kinematics extracted maximal pelvic tilt (PTmax), hip flexion (HFmax) and (mid-thoracic to lumbar) spinal flexion (SFmax). The SBR pelvic movement contribution (ΔPTrel) was calculated as ΔPT/(ΔPT+ΔPFA)∗100 for the radiographic analysis and as PTmax/(PTmax+HFmax) for the motion analyses. Axial and sagittal, pelvic and spinal range of motion (ROM) were calculated for STR and SBR, respectively. Spearman's rank-order determined correlations between the spinopelvic radiographs and sagittal kinematics, and the sagittal/axial kinematics. Mann-Whitney U-tests compared measures between groups.
Introduction
A common goal of total knee arthroplasty (TKA) is to restore normal knee kinematics. While substantial data is available on TKA kinematics, information regarding non-implanted knee kinematics is less well studied especially in larger patient populations. The objectives of this study were to determine normal femorotibial kinematics in a large number of non-implanted knees and to investigate parameters that yield higher knee flexion with weight-bearing activities.
Methods
Femorotibial kinematics of 104 non-implanted healthy subjects performing a deep knee bend (DKB) activity were analyzed using 3D to 2D fluoroscopy. The average age and BMI were 38.1±18.2 years and 25.2±4.6, respectively. Pearson correlation analysis was used to determine statistical correlations.

20 patients were treated with a calipered KA and a PCL retaining implant with a 1:1 medial ball-in-socket constraint and a non-constrained lateral flat articular insert surface. Verification checks, that are validated to restore native tibial compartment forces without release of healthy ligaments including the PCL, were used to select the optimal insert thickness. Trial inserts with thicknesses ranging from 10 to 13 mm were 3-D printed with medial goniometric markings that record rotation from 20° external to −20° internal with respect to a sagittal line laser marked on center of the medial condyle of the trial femoral component at maximum extension and 90° of flexion (Figure 1).
Introduction
Cementless total knee arthroplasty (TKA) implants use an interference fit to achieve fixation, which depends on the difference between the inner dimensions of the implant and outer dimensions of the bone. However, the most optimal interference fit is still unclear. A higher interference fit could lead to a superior fixation, but it could also cause bone abrasion and permanent deformation during implantation. Therefore, this study aims to investigate the effect of increasing the interference fit from 350 µm to 700 µm on the primary stability of cementless tibial implants by measuring micromotions and gaps at the bone-implant interface when subjected to two loading conditions.
Methods
Two cementless e.motion® tibial components (Total Knee System, B. Braun) with different interference fit and surface coating were implanted in six pairs of relatively young human cadaver tibias (47–60 years). The Orthoload peak loads of gait (1960N) and squat (1935N) were applied to the specimens with a custom made load applicator (Figure 1A). The micromotions (shear displacement) and opening/closing gaps (normal displacement) were measured with Digital Image Correlation (DIC) in 6 different regions of interest (ROIs - Figure 1B). Two General Linear Mixed Models (GLMMs) were created with micromotions and interfacial gaps as dependent variables, bone quality, loading conditions, ROIs, and interference fit implants as independent variables, and the cadaver specimens as subject variables.
Background
While posterior cruciate retaining (PCR) implants are a more common total knee arthroplasty (TKA) design, newer bi-cruciate retaining (BCR) TKAs are now being considered as an option for many patients, especially those that are younger. While PCR TKAs remove the ACL, the BCR TKA designs keep both cruciate ligaments intact, as it is believed that the resection of the ACL greatly affects the overall kinematic patterns of TKA designs. Various fluoroscopic studies have focused on determination of kinematics but haven't defined differentiators that affect motion patterns. This research study assesses the importance of the cruciate ligaments and femoral geometry for Bi-Cruciate Retaining (BCR) and Posterior Cruciate Retaining (PCR) TKAs having the same femoral component, compared to the normal knee.
Methods
The in vivo 3D kinematics were determined for 40 subjects having a PCR TKA, 10 having a BCR TKA, and 10 having a normal knee, in a retrospective study. All TKA subjects had the same femoral component. All subjects performed a deep knee bend under fluoroscopic surveillance. The kinematics were determined during early flexion (ACL dominant), mid flexion (ACL/PCL transition) and deep flexion (PCL dominant).
Purpose
Various alignment philosophies for total knee arthroplasty (TKA) have been described, all striving to achieve excellent long-term implant survival and good functional outcomes. In recent years, in search of higher functionality and patient satisfaction, a shift towards more patient-specific alignment is seen. Robotics is the perfect technology to tailor alignment. The purpose of this study was to describe ‘inverse kinematic alignment’ (iKA) technique, and to compare clinical outcomes of patients that underwent robotic-assisted TKA performed by iKA versus adjusted mechanical alignment (aMA).
Methods
The authors analysed the records of a consecutive series of patients that received robotic assisted TKA with iKA (n=40) and with aMA (n=40). Oxford Knee Score (OKS) and satisfaction on a visual analogue scale (VAS) were collected at a follow-up of 12 months. Clinical outcomes were assessed according to patient acceptable symptom state (PASS) thresholds, and uni- and multivariable linear regression analyses were performed to determine associations of OKS and satisfaction with 6 variables (age, sex, body mass index (BMI), preoperative hip knee ankle (HKA) angle, preoperative OKS, alignment technique).
Combined Partial Knee Arthroplasty (CPKA) is a promising alternative to Total Knee Arthroplasty (TKA) for the treatment of multi-compartment arthrosis. Through the simultaneous or staged implantation of multiple Partial Knee Arthroplasties (PKAs), CPKA aims to restore near-normal function of the knee, through retention of the anterior cruciate ligament and native disease-free compartment. Whilst PKA is well established, CPKA is comparatively novel and associated biomechanics are less well understood.
Clinically, PKA and CPKA have been shown to better restore knee function compared to TKA, particularly during fast walking. The biomechanical explanation for this superiority remains unclear but may be due to better preservation of the extensor mechanism. This study sought to assess and compare extensor function after PKA, CPKA, and TKA.
An instrumented knee extension rig facilitated the measurement extension moment of twenty-four cadaveric knees, which were measured in the native state and then following a sequence of arthroplasty procedures. Eight knees underwent medial Unicompartmental Knee Arthroplasty (UKA-M), followed by patellofemoral arthroplasty (PFA) thereby converting to medial Bicompartmental Knee Arthroplasty (BCA-M). In the final round of testing the PKA implants were removed a posterior-cruciate retaining TKA was implanted. The second eight received lateral equivalents (UKA-L then BCA-L) then TKA. The final eight underwent simultaneous Bi-Unicondylar Arthroplasty (Bi-UKA) before TKA. Extensor efficiencies over extension ranges typical of daily tasks were also calculated and differences between arthroplasties were assessed using repeated measures analysis of variance.
For both the medial and lateral groups, UKA demonstrated the same extensor function as the native knee. BCA resulted in a small reduction in extensor moment between 70–90° flexion but, in the context of daily activity, extensor efficiency was largely unaffected and no significant reductions were found. TKA, however, resulted in significantly reduced extensor moments, leading to efficiency deficits ranging from 8% to 43% in flexion ranges associated with downhill walking and the stance phase of gait, respectively.
Comparing the arthroplasties: TKA was significantly less efficient than both UKA-M and BCA-M over ranges representing stair ascent and gait; TKA showed a significant 23% reduction compared to BCA-L in the same range. There were no differences in efficiency between the UKAs and BCAs over any flexion range and TKA efficiency was consistently lower than all other arthroplasties.
Bi-UKA generated the same extensor moment as native knee at flexion angles typical of fast gait (0–30°). Again, TKA displayed significantly reduced extensor moments towards full extension but returned to the normal range in deep flexion. Overall, TKA was significantly less efficient following TKA than Bi-UKA.
Recipients of PKA and CPKA have superior functional outcomes compared to TKA, particularly in relation to fast walking. This in vitro study found that both UKA and CPKA better preserve extensor function compared to TKA, especially when evaluated in the context of daily functional tasks. TKA reduced knee extensor efficiency by over 40% at flexion angles associated with gait, arguably the most important activity to maintain patient satisfaction. These findings go some way to explaining functional deficiencies of TKA compared to CPKA observed clinically.
As treatments of knee osteoarthrosis are continually refined, increasingly sophisticated methods of evaluating their biomechanical function are required. Whilst TKA shows good preoperative pain relief and survivorship, functional outcomes are sub-optimal, and research focus has shifted towards their improvement. Restoration of physiological function is a common design goal that relies on clear, detailed descriptions of native biomechanics. Historical simplifications of true biomechanisms, for example sagittal plane approximation of knee kinematics, are becoming progressively less suitable for evaluation of new technologies. The patellar tendon moment arm (PTMA) is an example of such a metric of knee function that usefully informs design of knee arthroplasty but is not fully understood, in part due to limitations in its measurement. This research optimized PTMA measurement and identified the influence of knee size and sex on its variation.
The PTMA about the instantaneous helical axis was calculated from optical tracked positional data. A fabricated knee model facilitated calculation optimization, comparing four data smoothing techniques (raw, Butterworth filtering, generalized cross-validated cubic spline-interpolation and combined filtering/interpolation). The PTMA was then measured for 24 fresh-frozen cadaveric knees, under physiologically based loading and extension rates. Sex differences in PTMA were assessed before and after size scaling.
Large errors were measured for raw and interpolated-only techniques in the mid-range of extension, whilst both raw and filtered-only methods saw large inaccuracies at terminal extension and flexion. Combined filtering/interpolation enabled sub-mm PTMA calculation accuracy throughout the range of knee flexion, including at terminal extension/flexion (root-mean-squared error 0.2mm, max error 0.5mm) (Figure 1).
Before scaling, mean PTMA throughout flexion was 46mm; mean, peak, and minimum PTMA values were larger in males, as was the PTMA at terminal flexion, the change in PTMA from terminal flexion to peak, and the change from peak to terminal extension (mean differences ranging from 5 to 10mm, p<0.05). Knee size was highly correlated with PTMA magnitude (r>0.8, p<0.001) (Figure 2). Scaling eliminated sex differences in PTMA magnitude, but peak PTMA occurred closer to terminal extension in females (female 15°, male 29°, p=0.01) (Figure 3).
Improved measurement of the PTMA reveals previously undocumented characteristics that may help to improve the functional outcomes of knee arthroplasty. Knee size accounted for two-thirds of the variation in PTMA magnitude, but not the flexion angle at which peak PTMA occurred, which has implications for morphotype-specific arthroplasty and musculoskeletal models. The developed calculation framework is applicable both in vivo and vitro for accurate PTMA measurement and might be used to evaluate the relative performance of emerging technologies.

Introduction
Model-based radiostereometric analysis (MBRSA) allows the in vivo measurement of implant loosening (i.e. migration) from a host bone by acquiring a pair of biplanar radiographs of the patient's implant over time. Focusing on total knee replacement patients, the accuracy of MBRSA in calculating tibial baseplate migration depends on the accuracy in registering a 3D model onto the biplanar radiographs; thus, the shape of the baseplate and its orientation relative to the imaging planes is pertinent. Conventionally, the baseplate coordinate system is aligned with the laboratory coordinate system, however, this reference orientation is unnecessary and may hide unique baseplate features resulting in less accurate registration (Figure 1). Therefore, the primary objective of this study was to determine the optimal baseplate orientation for improving accuracy during MBRSA, and an acceptable range of orientations for clinical use. A second objective was to demonstrate that a custom knee positioning guide repeatably oriented the baseplate within the acceptable range of orientations.
Materials and Methods
A tibia phantom consisting of a baseplate rigidly fixed to a sawbone was placed in 24 orientations (combination of six rotations about X (i.e. knee flexion) and four rotations about Z (i.e. hip abduction)) with three pairs of radiographs acquired at each orientation. The radiographs were processed in MBRSA software, and the mean maximum total point motion (MTPM), an indicator of bias error during model registration, was plotted as a function of the two rotations to determine the optimal orientation and a range of acceptable orientations (Figure 2).
A custom knee positioning guide was manufactured with the goal of orienting the baseplate close to the optimal orientation and within the acceptable range of orientations (Figure 3). Ten independent pairs of biplanar radiographs were acquired by repeatedly placing a knee model in the knee positioning guide, and the images were processed in MBRSA software to determine the baseplate orientation.
Introduction
Robotics have been applied to total knee arthroplasty (TKA) to improve surgical precision in component placement and joint function restoration. The purpose of this study was to evaluate prosthetic component alignment in robotic arm-assisted (RA)-TKA performed with functional alignment and intraoperative fine-tuning, aiming for symmetric medial and lateral gaps in flexion/extension. It was hypothesized that functionally aligned RA-TKA the femoral and tibial cuts would be performed in line with the preoperative joint line orientation.
Methods
Between September 2018 and January 2020, 81 RA cruciate retaining (CR) and posterior stabilized (PS) TKAs were performed at a single center. Preoperative radiographs were obtained, and measures were performed according to Paley's. Preoperatively, cuts were planned based on radiographic epiphyseal anatomies and respecting ±3° boundaries from neutral coronal alignment. Intraoperatively, the tibial and femoral cuts were modified based on the individual soft tissue-guided fine-tuning, aiming for symmetric medial and lateral gaps in flexion/extension. Robotic data were recorded.
Introduction
Surgeons commonly resect additional distal femur during primary total knee arthroplasty (TKA) to correct a flexion contracture to restore range of motion and knee function. However, the effect of joint line elevation on the resulting TKA kinematics including frontal plane laxity is unclear. Thus, our goal was to quantify the effect of additional distal femoral resection on passive extension and mid-flexion laxity.
Methods
Six computational knee models with capsular and collateral ligament properties specific to TKA were developed and implanted with a contemporary posterior-stabilized TKA. A 10° flexion contracture was modeled by imposing capsular contracture as determined by simulating a common clinical exam of knee extension and accounting for the length and weight of each limb segment from which the models were derived (Figure 1). Distal femoral resections of 2 mm and 4 mm were simulated for each model. The knees were then extended by applying the measured knee moments to quantify the amount of knee extension. The output data were compared with a previous cadaveric study using a two-sample two-tailed t-test (p<0.05) [1]. Subsequently, varus and valgus torques of ±10 Nm were applied as the knee was flexed from 0° to 90° at the baseline, and after distal resections of 2 mm, and 4 mm. Coronal laxity, defined as the sum of varus and valgus angulation in response to the applied varus and valgus torques, was measured at 30° and 45°of flexion, and the flexion angle was identified where the increase in laxity was the greatest with respect to baseline.
Robotic-assisted technology in total knee arthroplasty (TKA) aims to increase implantation accuracy, with real-time data being used to estimate intraoperative component alignment. Postoperatively, Perth computed tomography (CT) protocol is a valid measurement technique in determining both femoral and tibial component alignments. The aim of this study was to evaluate the accuracy of intraoperative component alignment by robotic-assisted TKA through CT validation. A total of 33 patients underwent TKA using the MAKO robotic-assisted TKA system. Intraoperative measurements of both femoral and tibial component placements, as well as limb alignment as determined by the MAKO software were recorded. Independent postoperative Perth CT protocol was obtained (n.29) and compared with intraoperative values. Mean absolute difference between intraoperative and postoperative measurements for the femoral component were 1.17 degrees (1.10) in the coronal plane, 1.79 degrees (1.12) in the sagittal plane, and 1.90 degrees (1.88) in the transverse plane. Mean absolute difference between intraoperative and postoperative measurements for the tibial component were 1.03 degrees (0.76) in the coronal plane and 1.78 degrees (1.20) in the sagittal plane. Mean absolute difference of limb alignment was 1.29 degrees (1.25), with 93.10% of measurements within 3 degrees of postoperative CT measurements. Overall, intraoperatively measured component alignment as estimated by the MAKO robotic-assisted TKA system is comparable to CT-based measurements.
Introduction
In prosthetic knee surgery, the axis of the lower limb is often determined only by static radiographic analysis. However, it is relevant to determine if this axis varies during walking, as this may alter the stresses on the implants. The aim of this study was to determine whether pre-operative measurement of the mechanical femorotibial axis (mFTA) varies between static and dynamic analysis in isolated medial femorotibial osteoarthritis.
Methods
Twenty patients scheduled for robotic-assisted medial unicompartmental knee arthroplasty (UKA) were included in this prospective study. We compared three measurements of the coronal femorotibial axis: in a static and weightbearing position (on long leg radiographs), in a dynamic but non-weightbearing position (intra-operative acquisition during robotic-assisted UKA), and in a dynamic and weightbearing position (during walking by a gait analysis).
Introduction
Increasingly young and active patients are concerned about revision arthroplasty forcing the manufacturers to think about revision prostheses that fit to this population while meeting the indications and fitting with bone losses and ligament deficiencies. One of those industrials claims that its system allows the surgeon to rise the constraint from a posterior stabilized (PS) prostheses to a semi-constraint total stabilized (TS) prostheses without modifying the gait pattern thanks to a similar single radius design. The aim of the study was to compare gait parameters in patients receiving either PS or TS knee prostheses.
Methods
Nineteen patients in each groups were prospectively collected for this study and compared between each other. All subjects were assessed with a 3D knee kinematics analysis, performed with an optoelectronic knee assessment device (KneeKG®).
