We summarise and highlight the safety concerns
within the field of trauma and orthopaedic surgery with particular
emphasis placed on current controversies and reforms within the United
Kingdom National Health Service.
Mesenchymal stem-cell based therapies have been
proposed as novel treatments for intervertebral disc degeneration,
a prevalent and disabling condition associated with back pain. The
development of these treatment strategies, however, has been hindered
by the incomplete understanding of the human nucleus pulposus phenotype
and by an inaccurate interpretation and translation of animal to
human research. This review summarises recent work characterising
the nucleus pulposus phenotype in different animal models and in
humans and integrates their findings with the anatomical and physiological
differences between these species. Understanding this phenotype
is paramount to guarantee that implanted cells restore the native

Lower limb muscle power is thought to influence outcome following
total knee replacement (TKR). Post-operative deficits in muscle
strength are commonly reported, although not explained. We hypothesised
that post-operative recovery of lower limb muscle power would be
influenced by the number of satellite cells in the quadriceps muscle at
time of surgery. Biopsies were obtained from 29 patients undergoing TKR. Power
output was assessed pre-operatively and at six and 26 weeks post-operatively
with a Leg Extensor Power Rig and data were scaled for body weight.
Satellite cell content was assessed in two separate analyses, the
first cohort (n = 18) using immunohistochemistry and the second
(n = 11) by a new quantitative polymerase chain reaction (q-PCR)
protocol for Pax-7 (generic satellite cell marker) and Neural Cell
Adhesion Molecule (NCAM; marker of activated cells).Objectives

Continuing professional development (CPD) refers
to the ongoing participation in activities that keep a doctor up
to date and fit to practise once they have completed formal training.
It is something that most will do naturally to serve their patients
and to enable them to run a safe and profitable practice. Increasingly,
regulators are formalising the requirements for evidence of CPD,
often as part of a process of revalidation or relicensing. This paper reviews how orthopaedic journals can be used as part

Post-operative complications after total hip
or knee replacement can delay recovery, prolong hospitalisation, increase
rates of re-admission and, in the most severe cases, lead to long-term
disability or even death. In this analysis of pooled data from four
large, randomised, phase III clinical trials that compared the oral,
direct Factor Xa inhibitor rivaroxaban with subcutaneous enoxaparin
for the prevention of venous thromboembolism after total hip or
knee replacement (n = 12 729), the incidence of complications, including
bleeding and adverse events related to surgery (such as wound infection,
wound dehiscence and haemarthrosis) are reported. Interventions
and procedures relating to surgery are also compared between the
groups. Bleeding events, including excessive wound haematoma and
surgical-site bleeding, occurred at similar rates in the rivaroxaban
and enoxaparin groups. Over the total study duration, adverse surgical
events occurred at a similar rate in the rivaroxaban group compared
with the enoxaparin group after total knee replacement (2.26% This analysis shows that the incidence of adverse surgical events
with rivaroxaban was similar to enoxaparin.

The aim of this study was to review the role
of clinical trial networks in orthopaedic surgery. A total of two
electronic databases (MEDLINE and EMBASE) were searched from inception
to September 2013 with no language restrictions. Articles related
to randomised controlled trials (RCTs), research networks and orthopaedic
research, were identified and reviewed. The usefulness of trainee-led
research collaborations is reported and our knowledge of current
clinical trial infrastructure further supplements the review. Searching
yielded 818 titles and abstracts, of which 12 were suitable for
this review. Results are summarised and presented narratively under
the following headings: 1) identifying clinically relevant research
questions; 2) education and training; 3) conduct of multicentre
RCTs and 4) dissemination and adoption of trial results. This review
confirms growing international awareness of the important role research
networks play in supporting trials in orthopaedic surgery. Multidisciplinary
collaboration and adequate investment in trial infrastructure are crucial

The pathogenesis of rotator cuff disease (RCD) is complex and
not fully understood. This systematic review set out to summarise
the histological and molecular changes that occur throughout the
spectrum of RCD. We conducted a systematic review of the scientific literature
with specific inclusion and exclusion criteria.Introduction

in experimental animals. Further development may help fracture healing
in the clinical situation.
A total of 187 patients with primary osteoarthritis
(OA) of the knee undergoing total knee replacement (TKR) were randomly
divided into two groups, one of which underwent synovectomy. The
patients and assessors were blinded to the randomisation both before
and after surgery. The duration of surgery, hospitalisation period,
concealed bleeding, drainage volume, blood transfusion rate and
range of movement of the knee at three days after the operation
were analysed. Patients were followed up at four weeks and 12 months
after their operation, and a visual analogue score (VAS) for pain,
Knee Society score (KSS) and a patellar ballottement test were compared

Permanent growth arrest of the longer bone is
an option in the treatment of minor leg-length discrepancies. The
use of a tension band plating technique to produce a temporary epiphysiodesis
is appealing as it avoids the need for accurate timing of the procedure
in relation to remaining growth. We performed an animal study to
establish if control of growth in a long bone is possible with tension
band plating. Animals (pigs) were randomised to temporary epiphysiodesis
on either the right or left tibia. Implants were removed after ten
weeks. Both tibiae were examined using MRI at baseline, and after
ten and 15 weeks. The median interphyseal distance was significantly shorter
on the treated tibiae after both ten weeks (p = 0.04) and 15 weeks
(p = 0.04). On T1-weighted images the metaphyseal water
content was significantly reduced after ten weeks on the treated
side (p = 0.04) but returned to values comparable with the untreated
side at 15 weeks (p = 0.14). Return of growth was observed in all
animals after removal of implants. Temporary epiphysiodesis can be obtained using tension band plating.
The technique is not yet in common clinical practice but might avoid
the need for the accurate timing of epiphysiodesis. Cite this article:
Despite advances in contemporary hip and knee
arthroplasty, blood loss continues to be an issue. Though blood transfusion
has long been used to treat post-operative anemia, the associated
risks are well established. The objective of this article is to
present two practical and effective approaches to minimising blood
loss and transfusion rates in hip and knee arthroplasty: the use
of antifibrinolytic medications such as tranexamic acid and the
adoption of more conservative transfusion indications.

Stem cells are a key component of regenerative medicine strategies. Particular areas of musculoskeletal application include cartilage and bone regeneration in arthritis and trauma. There are several types of stem cell and this article will focus on the adult derived cells. The review includes current issues and future developments.
