In an initial randomised controlled trial (RCT)
we segregated 180 patients to one of two knee positions following total
knee replacement (TKR): six hours of knee flexion using either a
jig or knee extension. Outcome measures included post-operative
blood loss, fall in haemoglobin, blood transfusion requirements,
knee range of movement, limb swelling and functional scores. A second
RCT consisted of 420 TKR patients randomised to one of three post-operative
knee positions: flexion for three or six hours post-operatively,
or knee extension. Positioning of the knee in flexion for six hours immediately
after surgery significantly reduced blood loss (p = 0.002). There

widely accepted disease-specific patient-reported outcome measure,
but there is limited evidence regarding any long-term trends in
the score. We reviewed 5600 individual OKS questionnaires (1547
patients) from a prospectively-collected knee replacement database,
to determine the trends in OKS over a ten-year period following
total knee replacement. The mean OKS pre-operatively was 19.5 (95%
confidence interval (CI) 18.8 to 20.2). The maximum post-operative
OKS was observed at two years (mean score 34.4 (95% CI 33.7 to 35.2)),
following which a gradual but significant decline was observed through
to the ten-year assessment (mean score 30.1 (95% CI 29.1 to 31.1))

method of treating end-stage arthritis of the knee. It is not, however,
a procedure without risk due to a number of factors, one of which
is diabetes mellitus. The purpose of this study was to estimate
the general prevalence of diabetes in patients about to undergo
primary TKR and to determine whether diabetes mellitus adversely
affects the outcome. We conducted a systematic review and meta-analysis
according to the Meta-analysis Of Observational Studies in Epidemiology
(MOOSE) guidelines. The Odds Ratio (OR) and mean difference (MD)
were used to represent the estimate of risk of a specific outcome.
Our results showed the prevalence of diabetes mellitus among patients
undergoing TKR was 12.2%. Patients with diabetes mellitus had an increased
risk of deep infection (OR = 1.61, 95% confidence interval (CI),
1.38 to 1.88), deep vein thrombosis (in Asia, OR = 2.57, 95% CI,
1.58 to 4.20), periprosthetic fracture (OR = 1.89, 95% CI, 1.04
to 3.45), aseptic loosening (OR = 9.36, 95% CI, 4.63 to 18.90),
and a poorer Knee Society function subscore (MD = -5.86, 95% CI,
-10.27 to -1.46). Surgeons should advise patients specifically about
these increased risks when obtaining informed consent and be meticulous about

Total knee arthroplasty (TKA) is known to lead
to a reduction in periprosthetic bone mineral density (BMD). In theory,
this may lead to migration, instability and aseptic loosening of
the prosthetic components. Bisphosphonates inhibit bone resorption
and may reduce this loss in BMD. We hypothesised that treatment
with bisphosphonates and calcium would lead to improved BMD and
clinical outcomes compared with treatment with calcium supplementation
alone following TKA. A total of 26 patients, (nine male and 17 female,
mean age 67 years) were prospectively randomised into two study
groups: alendronate and calcium (bisphosphonate group, n = 14) or calcium
only (control group, n = 12). Dual energy X-ray absorptiometry (DEXA)
measurements were performed post-operatively, and at three months,
six months, one, two, four, and seven years post-operatively. Mean femoral metaphyseal BMD was significantly higher in the
bisphosphonate group compared with controls, up to four years following
surgery in some areas of the femur (p = 0.045). BMD was observed
to increase in the lateral tibial metaphysis in the bisphosphonate
group until seven years (p = 0.002), and was significantly higher than
that observed in the control group throughout (p = 0.024). There
were no significant differences between the groups in the central
femoral metaphyseal, tibial medial metaphyseal or diaphyseal regions
of interest (ROI) of either the femur or tibia. Bisphosphonate treatment after TKA may be of benefit for patients
with poor bone quality. However, further studies with a larger number
of patients are necessary to assess whether this is clinically beneficial. Cite this article:
We compared the alignment of 39 total knee replacements implanted using the conventional alignment guide system with 37 implanted using a CT-based navigation system, performed by a single surgeon. The knees were evaluated using full-length weight-bearing anteroposterior radiographs, lateral radiographs and CT scans. The mean hip-knee-ankle angle, coronal femoral component angle and coronal tibial component angle were 181.8° (174.2° to 188.3°), 88.5° (84.0° to 91.8°) and 89.7° (86.3° to 95.1°), respectively for the conventional group and 180.8° (178.2° to 185.1°), 89.3° (85.8° to 92.0°) and 89.9° (88.0° to 93.0°), respectively for the navigated group. The mean sagittal femoral component angle was 85.5° (80.6° to 92.8°) for the conventional group and 89.6° (85.5° to 94.0°) for the navigated group. The mean rotational femoral and tibial component angles were −0.7° (−8.8° to 9.8°) and −3.3° (−16.8° to 5.8°) for the conventional group and −0.6° (−3.5° to 3.0°) and 0.3° (−5.3° to 7.7°) for the navigated group. The ideal angles of all alignments in the navigated group were obtained at significantly higher rates than in the conventional group. Our results demonstrated significant improvements in component positioning with a CT-based navigation system, especially with respect to rotational alignment.

to work following TKA in patients of working age in the United Kingdom. Patients &
Methods. We prospectively assessed 289 patients (289 TKAs) aged ≤ 65 years
who underwent TKA between 2010 and 2013. There were 148 women. The
following were recorded pre-operatively: age, gender, body mass
index, social deprivation, comorbidities, indication for surgery,
work status and nature of employment, activity level as assessed by
the University of California, Los Angeles (UCLA) activity score
and Oxford Knee Score (OKS). The intention of patients to return
to work or to retire was not assessed pre-operatively. At a mean
of 3.4 years (2 to 4) post-operatively, the return to work status,
OKS, the EuroQol-5 dimensions (EQ-5D) score, UCLA activity score
and Work, Osteoarthritis and joint-Replacement (WORQ) score were
obtained. Univariate and multivariate analyses were performed. Results. Of 261 patients (90%) who were working before TKA, 105 (40%)
returned to any job, including 89 (34%) who returned to the same
job at a mean of 13.5 weeks (2 to 104) post-operatively. A total
of 108 (41%) retired following TKA and 18 remained on welfare. Patients
not working before the operation did not return to work. Median
UCLA scores improved in 125 patients (58%) from 4 (mild activity)

This study was undertaken to evaluate the relationships, interactions
and predictive capacity of variables available pre- and post-operatively
on patient satisfaction following TKR. Using nationally collected
patient reported outcome measures and data from the National Joint
Registry for England and Wales, we identified
22 798 patients who underwent TKR for osteoarthritis between August
2008 and September 2010. The ability of specific covariates to predict
satisfaction was assessed using ordinal logistic regression and
structural equational modelling. Only 4959 (22%) of 22 278 patients
rated the results of their TKR as ‘excellent’, despite the majority
(71%, n = 15 882) perceiving their knee symptoms to be much improved.

patterns, the overall migration of the tibial component in the study
group demonstrated a pattern of initial migration followed by stabilisation
after one year, with mean maximum total point motion (MTPM) of 0.02
mm (standard deviation (. sd. ) 0.20) between one and two years
post-operatively. The direction of greatest motion was subsidence,
which stabilised at three months post-operatively (mean translation

knee arthroplasty (UKA) and high tibial osteotomy (HTO) to total
knee arthroplasty (TKA). The aim of this study is to compare the
outcomes of revision surgery for the two procedures in terms of
complications, re-revision and patient-reported outcome measures (PROMs)
at a minimum of two years follow-up. Patients and Methods. This study was a retrospective review of data from an institutional
arthroplasty registry for cases performed between 2001 and 2014.
A total of 292 patients were identified, of which 217 had a revision
of HTO to TKA, and 75 had revision of UKA to TKA. While mean follow-up
was longer for the HTO group compared with the UKA group, patient
demographics (age, body mass index and Charlson co-morbidity index)
and PROMs (Short Form-36, Oxford Knee Score, Knee Society Score,
both objective and functional) were similar in the two groups prior
to revision surgery. Outcomes included the rate of complications
and
re-operation, PROMS and patient-reported satisfaction at six months
and two years post-operatively. We also compared the duration of
surgery and the need for revision implants in the two groups. . Results . At two-year follow-up, both groups of patients had made significant
improvement in terms of PROMs compared with pre-operative scores.

to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses

Peri-prosthetic patellar fracture following resurfacing
as part of total knee replacement (TKR) is an infrequent yet challenging
complication. This case-control study was performed to identify
clinical, radiological and surgical factors that increase the risk
of developing a spontaneous patellar fracture after TKR. Patellar
fractures were identified in 74 patients (88 knees) from a series
of 7866 consecutive TKRs conducted between 1998 and 2009. After excluding
those with a previous history of extensor mechanism realignment
or a clear traumatic event, a metal-backed patella, any uncemented
component or subsequent infection, the remaining 64 fractures were
compared with a matched group of TKRs with an excellent outcome
defined by the Knee Society score. The mean age of patients with
a fracture was 70 years (51 to 81) at the time of TKR. Patellar
fractures were detected at a mean of 13.4 months (2 to 84) after

Tranexamic acid (TXA) is an antifibrinolytic agent used as a blood-sparing technique in total knee arthroplasty (TKA), and is routinely administered by intravenous (IV) or intra-articular (IA) injection. Recently, a novel method of TXA administration, the combined IV and IA application of TXA, has been applied in TKA. However, the scientific evidence of combined administration of TXA in TKA is still meagre. This meta-analysis aimed to investigate the efficacy and safety of combined IV and IA TXA in patients undergoing TKA. Materials and Methods. A systematic search was carried out in PubMed, the Cochrane Clinical Trial Register (Issue12 2015), Embase, Web of Science and the Chinese Biomedical Database. Only randomised controlled trials (RCT) evaluating the efficacy and safety of combined use TXA in TKA were identified. Two authors independently identified the eligible studies, extracted data and assessed the methodological quality of included studies. Meta-analysis was conducted using Review Manager 5.3 software. Results. A total of ten RCTs (1143 patients) were included in this study. All the included studies were randomised and the quality of included studies still needed improvement. The results indicated that, compared with either placebo or the single-dose TXA (IV or IA) group, the combination of IV and IA TXA group had significantly less total blood loss, hidden blood loss, total drain output, a lower transfusion rate and a lower drop in haemoglobin level. There were no statistically significant differences in complications such as wound infection and deep vein thrombosis between the combination group and the placebo or single-dose TXA group. Aims. Our aim was to perform a meta-analysis of the outcomes of revision
anterior cruciate ligament (ACL) reconstruction, comparing the use
of different types of graft. Materials and Methods. A search was performed of Medline and Pubmed using the terms
“Anterior Cruciate Ligament” and “ACL” combined with “revision”,
“re-operation” and “failure”. Only studies that reported the outcome
at a minimum follow-up of two years were included. Two authors reviewed
the papers, and outcomes were subdivided into autograft and allograft. Autograft
was subdivided into hamstring (HS) and bone-patellar tendon-bone
(BPTB). Subjective and objective outcome measures were analysed
and odds ratios with confidence intervals were calculated. Results. A total of 32 studies met the inclusion criteria. Five studies
used HS autografts, eight reported using BPTB autografts, two used
quadriceps tendon autografts and eight used various types. Seven
studies reported using allografts, while the two remaining used
both BPTB autografts and allografts. Overall, 1192 patients with
a mean age of 28.7 years (22.5 to 39) and a mean follow-up of 5.4
years (2.0 to 9.6) were treated with autografts, while 269 patients
with a mean age of 28.4 years (25 to 34.6) and a mean follow-up
of 4.0 years (2.3 to 6.0) were treated with allografts. Regarding
allografts, irradiation with 2.5 mrad was used in two studies while
the graft was not irradiated in the seven remaining studies. Reconstructions
following the use of autografts had better outcomes than those using
allograft with respect to laxity, measured by KT-1000/2000 (MEDmetric
Corporation) and the rates of complications and re-operations. Those
following the use of allografts had better mean Lysholm and Tegner
activity scores compared with autografts. If irradiated allografts
were excluded from the analysis, outcomes no longer differed between
the use of autografts and allografts. Comparing the types of autograft,

(PSCG) or conventional instrumentation (CI). . The primary outcome measure was the mechanical axis, as measured
at three months on a standing long-leg radiograph by the hip–knee–ankle
(HKA) angle. This was undertaken by an independent observer who
was blinded to the instrumentation. Secondary outcome measures were
component positioning, operating time, Knee Society and Oxford knee
scores, blood loss and length of hospital stay. A total of 126 patients (67 in the CI group and 59 in the PSCG
group) had complete clinical and radiological data. There were 88
females and 52 males with a mean age of 69.3 years (47 to 84) and

identify sources of survivorship, KSS, and reasons for revision.
Survivorship results were compared with values in the National Joint
Registry of England, Wales, and Northern Ireland (NJR). Results. A total of eight studies that included data for 1146 TKRs performed
in six countries satisfied the inclusion/exclusion criteria. Pooled
component survivorship estimates were 99.2% (95% CI, 97.7 to 99.7)
and 97.6% (95% CI, 95.8 to 98.6) at five and eight years, respectively.

(MPFL) reconstruction using an autologous semitendinosus graft.
The technique includes superolateral portal arthroscopic assessment
before and after graft placement to ensure correct graft tension
and patellar tracking before fixation. Between October 2005 and
October 2010, a total of 201 consecutive patients underwent 219 procedures.
Follow-up is presented for 211 procedures in 193 patients with a
mean age of 26 years (16 to 49), and mean follow-up of 16 months
(6 to 42). Indications were atraumatic recurrent patellar dislocation
in 141 patients, traumatic recurrent dislocation in 50, pain with

study: 16 men (19.8%) and 65 women (80.2%). Of these, 62 patients

We compared inflammation in the knee after total knee replacement (TKR) for primary osteoarthritis between two groups of patients undergoing joint replacement with and without synovectomy. A total of 67 patients who underwent unilateral TKR were randomly divided into group I, TKR without synovectomy, and group II, TKR with synovectomy. We evaluated the outcome in a series of patients with recurrent patellar dislocation who had either medial transfer of the tibial tuberosity and lateral release or an isolated lateral release as the primary treatment. The decision to use one or other procedure was based on a pre-operative distance between the tibial tuberosity to the trochlear groove (TTTG) of less than 10 mm to include the tibial tuberosity transfer in addition to the lateral release. Between April 2002 and December 2006, 49 patients (63 knees) underwent one of these procedures. A total of 35 patients (46 knees) was evaluated at a mean of 38 months (13 to 71) post-operatively. Medial transfer of the tibial tuberosity was performed in 33 knees and isolated lateral release in the remaining 13. Evaluation included the International Knee Documentation Committee (IKDC), the Kujala and the Short-form 36 scores. From the tibial tuberosity group 23 knees also underwent radiological examination at follow-up. There were further episodes of patellar dislocation in six of the 46 knees available for review.
