We assessed the long-term outcomes of a large cohort of patients who have undergone a periacetabular osteotomy (PAO), and sought to validate a patient satisfaction questionnaire for use in a PAO cohort. All patients who had undergone a PAO from July 1998 to February 2013 were surveyed, with several patient-reported outcome measures (PROMs) and radiological measurements of preoperative acetabular dysplasia and postoperative correction also recorded. Patients were asked to rate their level of satisfaction with their operation in achieving pain relief, restoration of activities of daily living, ability to perform recreational activity, and their overall level of satisfaction with the procedure. Both cements were assumed to behave in a biomechanically similar fashion to the original cement. The primary aim of this study was to compare the migration of a polished triple-tapered femoral stem fixed with either Refobacin Bone Cement R or Palacos R + G bone cement. Repeated radiostereometric analysis was used to measure migration of the femoral head centre. The secondary aims were evaluation of cement mantle, stem positioning, and patient-reported outcome measures. Methods. Overall, 75 patients were included in the study and 71 were available at two years postoperatively. Aims. Acute and chronic injuries of the interosseus membrane can result in longitudinal instability of the forearm. Reconstruction of the central band of the interosseus membrane can help to restore biomechanical stability. Different methods have been used to reconstruct the central band, including tendon grafts, bone-ligament-bone grafts, and synthetic grafts. This Idea, Development, Exploration, Assessment, and Long-term (IDEAL) phase 1 study aims to review the clinical results of reconstruction using a synthetic braided cross-linked graft secured at either end with an Endobutton to restore the force balance between the bones of the forearm. Methods. An independent retrospective review was conducted of a consecutive series of 21 patients with longitudinal instability injuries treated with anatomical central band reconstruction between February 2011 and July 2019. Patients with less than 12 months’ follow-up or who were treated acutely were excluded, leaving 18 patients in total. Preoperative clinical and radiological assessments were compared with prospectively gathered data using range of motion and the abbreviated version of the Disabilities of the Arm, Shoulder and Hand questionnaire (QuickDASH) functional outcome score. Results. Of the 18 patients (nine male, nine female) who met the inclusion criteria, the median follow-up was 8.5 years (interquartile range (IQR) 5.6 to 10). Their mean age was 49 years (SD 11). The mean extension improved significantly from 38° (SD 15°) to 24° (SD 9°) (p = 0.027), with a mean flexion-extension arc change from 81° (SD 27°) to 93° (SD 30°) (p = 0.172) but with no forearm rotational improvement (p = 0.233) at latest follow-up. The QuickDASH functional score improved significantly from 80 (SD 14) to 52 (SD 26) following reconstruction (p = 0.031), but generally the level of disability remains high. Management of acetabular fractures in the elderly population remains somewhat controversial in regards to when to consider is open reduction internal fixation (ORIF) versus acute primary total hip. study aims to (1) describe outcome of this complex problem and investigate predictors of successful outcome. This retrospective study analyzes all acetabular fractures in patients over the age of 60, managed by ORIF at a tertiary trauma care centre between 2007 and 2018 with a minimum follow up of one year. Of the 117 patients reviewed, 85 patients undergoing ORIF for treatment of their acetabular fracture were included in the analysis. The remainder were excluded based management option including acute ORIF with THA (n=10), two-stage ORIF (n=2), external fixator only (n=1), acute THA (n=1), and conservative management (n=1). The remainder were excluded based on inaccessible medical records (n=6), mislabelled diagnosis (n=6), associated femoral injuries (n=4), acetabular fracture following hemiarthroplasty (n=1). The mean age of the cohort is 70±7 years old with 74% (n=62) of patients being male. Data collected included: demographics, mechanism of injury, Charlson Comorbidity Index (CCI), ASA Grade, smoking status and reoperations. To evaluate if, for orthopaedic trainees, additional cadaveric simulation training or standard training alone yields superior radiological and clinical outcomes in patients undergoing dynamic hip screw (DHS) fixation or hemiarthroplasty for hip fracture. Methods. This was a preliminary, pragmatic, multicentre, parallel group randomized controlled trial in nine secondary and tertiary NHS hospitals in England. Researchers were blinded to group allocation. Overall, 40 trainees in the West Midlands were eligible: 33 agreed to take part and were randomized, five withdrew after randomization, 13 were allocated cadaveric training, and 15 were allocated standard training. Introduction. The direct anterior approach (DAA) for total hip arthroplasty (THA) has gained popularity in recent years. Potential advantages over other surgical approaches include less postoperative pain, fewer postoperative precautions, and quicker early recovery. This aim of this study was to assess the reliability and validity of the Unified Classification System (UCS) for postoperative periprosthetic femoral fractures (PFFs) around cemented polished taper-slip (PTS) stems. Methods. Radiographs of 71 patients with a PFF admitted consecutively at two centres between 25 February 2012 and 19 May 2020 were collated by an independent investigator. Six observers (three hip consultants and three trainees) were familiarized with the UCS. Each PFF was classified on two separate occasions, with a mean time between assessments of 22.7 days (16 to 29). Treatment of large bone defects represents a great challenge for orthopedic surgeons. The main causes are congenital abnormalities, traumas, osteomyelitis and bone resection due to cancer. Aims. Uncemented metal acetabular components show good osseointegration, but material stiffness causes stress shielding and retroacetabular bone loss. Cemented monoblock polyethylene components load more physiologically; however, the cement bone interface can suffer fibrous encapsulation and loosening. It was hypothesized that an uncemented titanium-sintered monoblock polyethylene component may offer the optimum combination of osseointegration and anatomical loading. Methods. A total of 38 patients were prospectively enrolled and received an uncemented monoblock polyethylene acetabular (pressfit) component. This single cohort was then retrospectively compared with previously reported randomized cohorts of cemented monoblock (cemented) and trabecular metal (trabecular) acetabular implants. The primary outcome measure was periprosthetic bone density using dual-energy x-ray absorptiometry over two years. Secondary outcomes included radiological and clinical analysis. Results. Although there were differences in the number of males and females in each group, no significant sex bias was noted (p = 0.080). Furthermore, there was no significant difference in age (p = 0.910) or baseline lumbar bone mineral density (BMD) (p = 0.998) found between any of the groups (pressfit, cemented, or trabecular). The pressfit implant initially behaved like the trabecular component with an immediate fall in BMD in the inferior and medial regions, with preserved BMD laterally, suggesting lateral rim loading. However, the pressfit component subsequently showed a reversal in BMD medially with recovery back towards baseline, and a continued rise in lateral BMD. This would suggest that the pressfit component begins to reload the medial bone over time, more akin to the cemented component. Analysis of

Introduction. Malposition of the acetabular component in total hip arthroplasty (THA) is linked to multiple adverse outcomes. Changes in the sagittal plane position of the pelvis, owing both to patient positioning in the operating room and to altered spinopelvic alignment following surgery, potentially contribute to variation in component position. The dynamics of sagittal plane pelvic position before, during, and after THA have not been defined. We measured the differences in pelvic ratio, a measure of sagittal plane pelvic position, between preoperative, intraoperative, and postoperative anteroposterior (AP) radiographs of patients undergoing THA in the lateral decubitus position. Methods. We retrospectively compared the radiographic pelvic ratio among 90 patients undergoing THA. AP radiographs were obtained in the standing position preoperatively and at 6 weeks after surgery; in the lateral decubitus position after trial reduction intraoperatively; and in the supine position in the post anesthesia care unit (PACU). Pelvic ratio was defined as the ratio between the vertical distance from the inferior sacroiliac (SI) joints to the superior pubic symphysis and the horizontal distance between the inferior SI joints. Radlink software was used to determine the pelvic ratio on each radiograph. Changes in apparent cup position based on changes in pelvic ratio were calculated using data from the literature, and a change of at least 10 degrees in acetabular component position was defined as clinically meaningful. Analyses were performed using paired t-tests, with p<0.05 defined as significant. Results. 54% of patients had a change in pelvic ratio large enough to alter the apparent acetabular component anteversion by 10 degrees (49% increased and 6% decreased), and 12% had a change large enough to alter the apparent acetabular component inclination by 10 degrees (12% increased and 0% decreased) when the intraoperative AP radiograph was compared to the preoperative AP radiograph. 36% of patients had a change in pelvic ratio from the preoperative radiograph to the 6 week preoperative radiograph large enough to alter the apparent acetabular component anteversion by 10 degrees (5% increased and 31% decreased), and 8% had a change large enough to alter the apparent acetabular component inclination by 10 degrees (6% increased and 1% decreased). Abstract. OBJECTIVES. Valgus high tibial osteotomy (HTO) represents an effective treatment for patients with medial compartment osteoarthritis (OA) in a varus knee. However, the mechanisms which cause this clinical improvement are unclear. Previous studies suggest a wider stance gait can reduce medial compartment loading via reduction in the external knee adduction moment (KAM); a measure implicated in progression of medial compartment OA. This study aimed to measure whether valgus HTO is associated with a postoperative increase in static stance width. METHODS. 32 patients, recruited in the Biomechanics and Bioengineering Centre Versus Arthritis HTO study, underwent valgus (medial opening wedge) HTO. Weightbearing pre- and post- operative radiographs were taken showing both lower limbs. The horizontal distance, measured from a fixed point on the right talus to the corresponding point on the left, was divided by the talus width to give a standardised “stance width” for each radiograph. The difference between pre- and post- operative stance width was compared for each patient using a paired sample t-test. RESULTS. Preoperatively, mean stance was 4.00 talar-widths but postoperatively this increased to 5.41. This mean increase of 1.42 talar-widths was statistically significant (p=0.001) and represents a mean proportional increase in stance width of 35.5% following HTO. Of the 32 patients, 23 showed increased stance width and 9 decreased (range −4.64 to 6.00 talar-widths). CONCLUSIONS. Satisfactory intermediate and long-term results of rotational acetabular osteotomy (RAO) for the treatment of early osteoarthritis secondary to developmental dysplasia of the hip have been reported. The purpose of this study is to examine the 30-year results of RAO. Between 1987 and 1994, we treated 49 patients (55 hips) with RAO for diagnosis of pre- OA or early-stage OA. Of those patients, 35 patients (43 hips) were available at a minimum of 28 years. The follow-up rate was 78.2% and the mean follow-up was 30.5 years. The mean age at the time of surgery was 34 years. Aims. The aims of this study were to determine the change in pelvic sagittal alignment before, during, and after total hip arthroplasty (THA) undertaken with the patient in the lateral decubitus position, and to determine the impact of these changes on acetabular component position. Patients and Methods. We retrospectively compared the radiological pelvic ratio among 91 patients undergoing THA. In total, 41 patients (46%) were female. The mean age was 61.6 years (. sd. 10.7) and the mean body mass index (BMI) was 20.0 kg/m. 2. (. sd. 5.5). Anteroposterior radiographs were obtained: in the standing position preoperatively and at six weeks postoperatively; in the lateral decubitus position after trial reduction intraoperatively; and in the supine position in the post-anaesthesia care unit. Pelvic ratio was defined as the ratio between the vertical distance from the inferior aspect of the sacroiliac (SI) joints to the superior pubic symphysis and the horizontal distance between the inferior aspect of the SI joints. Changes in the apparent component position based on changes in pelvic ratio were determined, with a change of > 5° considered clinically significant. Analyses were performed using Wilcoxon’s signed-rank test, with p < 0.05 considered significant. Results. Introduction. Orientation of the acetabular component in total hip arthroplasty has been shown to influence component wear, stability, and impingement. Freehand placement of the component can lead to widely variable radiographic outcomes. Accurate abduction, in particular, can be difficult in the lateral decubitus position due to limited ability to appreciate and control positional obliquity of the pelvis. A CT-based mechanical navigation device has been shown to decrease cup placement error. This is an independent report of a single-surgeon's radiographic results using the device to control cup abduction. Patients and Methods. Sixty-four (64) consecutive elective THRs in 58 patients were performed via a supercapsular percutaneously-assisted (SuperPATH) surgical approach. Digital radiographs have taken over from conventional radiographs in most of the hospitals in New Zealand. This has created a challenge with respect to templating and pre-operative planning of total hip replacement surgeries. Digital templating has not, until recently, been available in our hospitals. Recently, a digital templating system (Cedara) has become available and has been installed and used at Middlemore Hospital and at Manukau Surgical Centre. This system allows computerised templating of digital radiographs. The aim of this study was to assess the accuracy of digital templating and to compare this method to the “compromised” conventional templating that has been performed at Middlemore hospital for the last 10 years. In order to correct for magnification a fiducial stand has been created. This is a plastic stand and a pole with a movable 36mm metal ball. The ball is placed at the level of the greater trochanter and the stand is then placed between the patients legs. A standard templating “AP pelvis for hips” radiograph is then taken with the limbs internally rotated, such that the patellae are facing anteriorly. A traditional hard film was then created from this image for traditional acetate templating. Digital templating, with the Cedara system, was performed on the digital images after calibrating the image size using the fiducial image of known size. The results of the two methods were tabulated. Slipped capital femoral epiphysis (SCFE) is traditionally treated with in situ fixation using a threaded screw, leading to physeal arrest while stabilizing the femoral head. Recently, there has been interest in alternative methods of fixation for SCFE, aiming to allow growth and remodelling of the femoral neck postoperatively. One such option is the Free Gliding SCFE Screw (Pega Medical), which employs a telescopic design intended to avoid physeal compression. Mechanical failure of spine posterior fixation in the lumbar region Is suspected to occur more frequently when the sagittal balance is not properly restored. While failures at the proximal extremity have been studied in the literature, the lumbar distal junctional pathology has received less attention. The aim of this work was to investigate if the spinopelvic parameters, which characterize the sagittal balance, could predict the mechanical failure of the posterior fixation in the distal lumbar region. All the spine surgeries performed in 2017-2019 at Rizzoli Institute were retrospectively analysed to extract all cases of lumbar distal junctional pathology. All the revision surgeries performed due to the pedicle screws pull-out, or the breakage of rods or screws, or the vertebral fracture, or the degenerative disc disease, in the distal extremity, were included in the junctional (JUNCT) group. A total of 83 cases were identified as JUNCT group. All the 241 fixation surgeries which to date have not failed were included in the control (CONTROL) group. Introduction. Dislocated hip hemiarthroplasties (HA) are associated with a 45% revision rate and 40% mortality rate. Implant selection for HA operations vary with no universally accepted implant choice. The WHiTE3 trial suggested older designs such as the Thompson has equitable outcomes to more modern and expensive implants such as the Exeter V40+Unitrax. Background. We occasionally came across cortical atrophy of femurs with cemented collarless polished triple-taper stem in a short term period. This study aimed to estimate radiographs of cemented collarless polished triple-taper stem taken 6 months after the initial operation. Methods. Between May 2009 and April 2011, 97 consecutive patients underwent primary total hip arthroplasty and hemiarthroplasty using SC-stem or C-stem implants. Background. Failed ingrowth and subsequent separation of revision acetabular components from the inferior hemi-pelvis constitutes a primary mode of failure in revision total hip arthroplasty (THA). Few studies have highlighted other techniques than multiple screws and an ischial flange or hook of cages to reinforce the ischiopubic fixation of the acetabular components, nor did any authors report the use of porous metal augments in the ischium and/or pubis to reinforce ischiopubic fixation of the acetabular cup. The aims of this study were to introduce the concept of extended ischiopubic fixation into the ischium and/or pubis during revision total hip arthroplasty [Fig. 2], and to determine the early clinical outcomes and the radiographic outcomes of hips revised with inferior extended fixation. Methods. Patients who underwent revision THA utilizing the surgical technique of extended ischiopubic fixation with porous metal augments secured in the ischium and/or pubis in a single institution from 2014 to 2016 were reviewed. 16 patients were included based on the criteria of minimum 24 months clinical and radiographic follow-up. No patients were lost to follow-up. The median duration of follow-up for the overall population was 37.43 months. The patients' clinical results were assessed using the Harris Hip Score (HHS), Western Ontario and McMaster Universities Osteoarthritis (WOMAC) index and Short form (SF)-12 score and satisfaction level based on a scale with five levels at each office visit. Factors such as the magnitude of the preoperative coronal curve, the use of hooks, number of levels fused, preoperative kyphosis, screw density, and rod type have all been implicated. INTRODUCTION. Acetabular cup malpositioning has been implicated in instability and wear-related complications after total hip arthroplasty. Although computer navigation and robotic assistance have been shown to improve the precision of implant placement, most surgeons use mechanical and visual guides to place acetabular components. Authors have shown that, when using a bean bag positioner, mechanical guides are misleading as they are unable to account for the variability in pelvic orientation during positioning and surgery. However, more rigid patient positioning devices may allow for more accurate free hand cup placement. To our knowledge, no study has assessed the ability of rigid devices to afford surgeons with ideal pelvic positioning throughout surgery. The purpose of this study is to utilize robotic-arm assisted computer navigation to assess the reliability of pelvic position in total hip arthroplasty performed on patients positioned with rigid positioning devices. METHODS. 100 hips (94 patients) prospectively underwent total hip Makoplasty in the lateral decubitus position from the posterior approach; 77 stabilized by universal lateral positioner, and 23 by peg board. After dislocation but prior to reaming, one fellowship trained arthroplasty surgeon manually placed the robotic arm parallel to both the longitudinal axis of the patient and the horizontal surface of the operating table, which, if the pelvis were oriented perfectly, would represent 0 degrees of anteversion and 0 degrees of inclination. The CT-templated computer software then generated true values of this perceived zero degrees of anteversion and inclination based on the position of the robot arm registered to a preoperative pelvic CT. Therefore, variations in pelvic positioning are represented by these robotic navigation generated values. To assure the accuracy of robotic measurements, cup anteversion and inclination at times of impaction were recorded and compared to those calculated via the trigonometric ellipse method of Lewinnek on standardized 3 months postoperative X-rays. RESULTS. Mean alteration in anteversion and inclination values were 1.7 degrees (absolute value 5.3 degrees, range −20 – 20 degrees) and 1.6 degrees (absolute value 2.6 degrees, range −8 – 10 degrees) respectively. 22% of anteversion values were altered by >10 degrees; 41% by > 5 degrees. There was no difference between positioners (p=0.36) and regression analysis revealed that anteversion differences were correlated with BMI (p=0.02). The use of endoprosthesis implants is frequent for tumours involving the proximal third of the femur and not amenable to primary arthroplasty or internal fixation. In this population, these implants are preferentially cemented given poor bone quality associated with systemic diseases and treatments. Loosening is a common complication of these implants that have been linked to poor bone quality, type of implants and importantly cementing technique. Thus, these techniques vary between different surgeons and based mainly on previous experience. One of the most successful cementing techniques in the arthroplasty literature is the French paradox. This technique involves removing the cancellous bone of the proximal femoral metaphysis and selects the largest stem to tightly fit the created cavity delineated by cortical bone. Cementing the implant results in a very thin cement layer that fills the inconsistent gaps between the metal and the bone. To our knowledge, no previous report exists in the literature assessing loosening in proximal femur replacement using the French paradox cementing technique. In this study, we sought to examine (1) rates of loosening in proximal femur replacement, and (2) the oncological outcomes including tumour recurrence and implant related complications. Objective. The use of all pedicle screw constructs for the management of spinal deformities has gained widespread popularity. However, the placement of pedicle screws in the deformed spine poses unique challenges for the spinal surgeon. The purpose of this study was to evaluate the complications and radiological outcomes of surgery in 124 consecutive patients with spinal deformity. These patients underwent correction of coronal and sagittal imbalance with segmental pedicle screw fixation only. Background. All pedicle screw constructs have been associated with improved correction in all three planes. In patients with severe deformity, such constructs can obviate the need for anterior surgeries, and the higher implant cost is offset by the avoidance of dual anterior and posterior approaches. Pedicle screw fixation enables enhanced correction of spinal deformities, but the technique is still not widely applied for thoracic deformities for fear of neurological complications. CT-generated pre-operative plans and cutting guides show promising results in terms of placement enhancement and reproducibility in clinical studies. The Medial Patellofemoral Ligament (MPFL) is the main restraining force against lateral patellar displacement. It is often disrupted following patellar subluxation or dislocation. MPFL reconstruction is frequently performed when conservative management fails and the patient experiences recurrent patellar dislocation. Various MPFL reconstruction procedures have been described in the literature and reported outcomes are encouraging. This study analyses the radiographic outcomes following MPFL reconstruction. From January 2006 to January 2011, 76 consecutive patients (80 knees) with patellar recurrent dislocation underwent medial patellofemoral ligament reconstruction in three large teaching hospitals. Mean follow-up was 31.8 months (range, 13–72). Semitendinosus and gracilis autografts were used for the reconstruction and all procedures were carried out by the senior authors (WL, BR, CW, MB). Plain radiographs (Anteroposterior (AP), Lateral and Skyline) performed preoperatively and postoperatively were used to compare the sulcus angle, congruence angle, lateral patellofemoral angle, trochlear dysplasia (Dejour classification), trochlear boss height and patellar height (Caton-Deschamps ratio). Plain radiographs (Lateral) performed postoperatively were used to evaluate the femoral tunnel placement used for MPFL reconstruction. The sulcus angle improved from 143.2° (122.9–157.7) to 139.3° (115.7–154.6) and the congruence angle improved from 26.7° (−17.5–82.6) to 0.26° (−35.3–7.8). The lateral patellofemoral angle was 7.99° (3.2–19.2) preoperatively and 9.02° (3.2–18.2) postoperatively. Purpose. Anterior column screw fixation has been a useful tool in the management of acetabular fractures, either alone or in combination with other fixation techniques. Percutaneous insertion may be advantageous by limiting surgical dissection but little has been reported on its safety. The purpose of this study is to report on the efficacy and safety of percutaneous anterior column stabilization. Method. In a consecutive series of 122 operatively treated acetabular fractures, 56 patients were treated with antegrade percutaneous anterior column stabilization either alone or in combination with other fixation techniques by a single surgeon (JY). The technique was selected when the anterior column portion of the fracture was undisplaced or could be reduced via indirect methods. We have looked at long-term results of unlinked Kudo 5 total elbow replacement used in the treatment of patients with rheumatoid arthritis in 2 hospitals. Methods: 87 Kudo 5 Total elbow replacements in 70 patients with adult rheumatoid arthritis were performed at Wexham Park Hospital, Slough and City Hospital, Nottingham by 2 specialist elbow surgeons, the senior authors. 16 patients had died and 8 patients were lost to follow up. 62 elbow replacements in 46 patients were evaluated at a mean follow up of 79 months [29–137 months] using the Mayo Clinic Performance Index.
For radiographic assessment of THA, we must estimate a 3-D structure with 2-D images. Basically, it has been good. But even after a successful surgery, sometimes we encountered an undersized stem in radiograph. Interestingly, it was more frequent after we introduced surgical robot for primary THA. It sometimes brought a huge dilemma during planning and evaluating the surgery. We performed this study to elucidate the cause of this problem. We used image data of 30 consecutive THAs using ROBODOC (ISS, USA). Aim: To assess the radiological appearance and subsequent behaviour of impacted allograft in revision hip surgery using the Charnley femoral prosthesis. Method: A prospective radiological study of hip arthroplasty revised for aseptic loosening with femoral bone loss was performed. Preoperative bone loss was assessed using the Endo Klinik grading. Impaction grafting with fresh frozen femoral head allograft and the Charnley stem was used in all cases. Postoperative X-rays and annual review films were examined for graft distribution, cortical repair and stem subsidence. Graft consolidation and cortical repair was assessed. Results: Twenty cases were followed up for 6 to 8 years. Fourteen cases had even distribution of graft and 6 had deficiency in one Gruen zone. Graft consolidation was identified in all cases at one year. Cortical repair was noted in 14 out of 15 Endo Klinik III cases. One patient died after 2 years and 9 months. Two stems have subsided by more than 5 mm and are asymptomatic. Two cases with subsidence have been revised. All cases with subsidence were associated with graft deficiency in Gruen zone II or III on the
Aims. Porous metaphyseal cones can be used for fixation in revision total knee arthroplasty (rTKA) and complex TKAs. This metaphyseal fixation has led to some surgeons using shorter cemented stems instead of diaphyseal engaging cementless stems with a potential benefit of ease of obtaining proper alignment without being beholden to the diaphysis. The purpose of this study was to evaluate short term clinical and radiographic outcomes of a series of TKA cases performed using 3D-printed metaphyseal cones. Methods. A retrospective review of 86 rTKAs and nine complex primary TKAs, with an average age of 63.2 years (SD 8.2) and BMI of 34.0 kg/m. 2. (SD 8.7), in which metaphyseal cones were used for both femoral and tibial fixation were compared for their knee alignment based on the type of stem used. Overall, 22 knees had cementless stems on both sides, 52 had cemented stems on both sides, and 15 had mixed stems.

Background The magnitude of the medial offset and the limb length discrepancy after a total hip replacement (THR) significantly alters the biomechanics of the hip. If both these components are not properly restored, the rate of dislocation may increase. Further decreased offset may result in impingement at the extremes of movement, and also results in soft-tissue laxity, while increased offset increases stress within the stem that may lead to stem fracture or loosening. Aims. There is little information about how to manage patients with a recurvatum deformity of the distal tibia and osteoarthritis (OA) of the ankle. The aim of this study was to evaluate the functional and radiological outcome of addressing this deformity using a flexion osteotomy and to assess the progression of OA after this procedure. Patients and Methods. A total of 39 patients (12 women, 27 men; mean age 47 years (28 to 72)) with a distal tibial recurvatum deformity were treated with a flexion osteotomy, between 2010 and 2015. Occasionally even though bony union occurs excessive lag screw sliding can cause persistent pain and soft tissue irritation and return to surgery for hardware removal. The purpose of this study was to evaluate if fracture stability, lag screw tip-apex distance (TAD), and quality of reduction have any impact excessive lag screw sliding and potential cutout. Methods. As part of our level one trauma center's institutional hip fracture registry, a retrospective analysis identified 199 intertrochanteric fractures fixed with CMN between 2009 and 2015 with follow up to union or a minimum of three months. The mean follow-up was 22 months (3 to 94 months). Mean patient age was 75 years (50 to 97 years) and 72% were women.

Patients who have multiple sclerosis (MS) may be at increased risk of developing complications after total hip arthroplasty (THA). The purpose of this study was to compare: 1) implant survivorship; 2) functional outcomes; 3) complication rates; and 4) radiographic findings after THA between MS patients and a matched cohort. A single institutional database was reviewed for patients who had a diagnosis of MS and underwent a THA. Thirty-four patients (41 hips) were matched to a 2:1 cohort who did not have MS using based on age, sex, body mass index (BMI), and Charlson/Deyo scores. This resulted in a matching cohort of 80 patients (82 hips). The available medical records were reviewed. Functional outcomes and complications were assessed.
Objectives/Background: Long-term outcome in THR is multifactorial influenced. Malpositioning leads to complications as early loosening, leg length difference or dislocation of the artificial joint. A proven factor for early losening is a misplacment of the center of rotation or varusposition of the stem. A higher luxation risk results out of high inclination and anteversion angles an reduced soft tissue tension. Aim of this study was to prove the image free navigation software in the modular Navitrack-System to check the implant position based on anatomical intraoperative acquired data. Design/Methods: In 35 consecutive cases we implantet the ACA screwcup and the Optan anatomical stem with use of the Navitrack navigation System. The software calculates al relevant data out of intraopertive probed landmarks. Intraoperative the shifting of the center of rotation, the cup anteversion and inclination, the stem inclination and torsion and the leglength shift is shown in real time. Intraoperative data were compared with postoperative position measurement in plain X-rays and in 15 patients in postoperative CT scans. Results: In all cases navigated implantpositioning was possible. In 3 cases stem positioning was not possible cause of refference loosening. The purposes of this study were to review retrospectively the 10-year outcome of cementless total hip arthroplasty (THA) using an active robot system in the femoral canal preparation for an anatomic short stem and navigation in the cup placement through a mini incision posterior approach. We reviewed all patients who underwent THA with this procedure in 53 hips between 2004 and 2007. There were no intraoperative fracture nor navigation- or robotic-related complications. All implant sizes were same as planned ones. All cases were followed up at least two years and all implants showed bone ingrowth stable according to the Engh's criteria. After then, six patients died of unrelated causes. Two patients (three hips) could not come to the 10-year follow-up examination. The remaining 44 hips were followed for 10 to 12 years (11 years on average). There is no dislocation. The average JOA hip score improved from 48 preoperatively to 96 at the final examination. On the
Purpose: Surgical prosthetic treatment of excentred degeneration of the shoulder joint has long been limited to simple humeral prostheses. Inverted total shoulder arthroplasty (ITSA) using the Grammont system appears to provide real improvement. What is the comparative outcome? Does the Grammont prosthesis give constant results? Is the simple humeral prosthesis still indicated for selected cases? In order to respond to these questions, we reviewed 84 patients who had undergone surgery between 1986 and 2000 for implantation of 52 simple humeral prostheses (mean follow-up 7.1 years) and 32 ITSA (mean follow-up 4.3 years). Material and methods: Between 1986 and 1995, all patients with excentred degeneration of the shoulder joint were treated with a simple humeral prosthesis (36 cases). Between 1995 and 2000, either ITSA or simple humeral prosthesis (16 cases) was used. For the ITSA, indications were limited to excentred degeneration in patients aged over 70 years with a painful pseudoparalytic shoulder or contraindication for a simple prosthesis (alteration of the acromiocorocoid vault or the subscapular area). Simple prostheses were used either when an ITSA could not be implanted (patient too young, overly deteriorated glenoid, deltoid insufficiency) or when there was a preferential indication (excentred degeneration in young patients <

When inserting a femoral stem, surgeons make use of many visual and tactile cues to be sure that the implant is correctly sized and well-seated. One such cue is the change of pitch that can be heard when the final femoral broach is inserted. This is known to be important, but has not been widely studied. We set out to analyse the sounds produced during femoral broaching and implant fixation, and to discover whether the absence of these sounds could predict a poor fixation. We recorded the sound of femoral broaching and definitive implant insertion, for twenty un-cemented Corail total hip replacements. Procedures were performed by the same surgeon, in the same theatre. The recordings were visualised using audio editing software, and a Fast Fourier Transform was used to identify the dominant audio frequencies. In 19 of the 20 cases, the final strikes of the final femoral broach displayed a distinctive pattern, with the most prominent frequencies being harmonics (multiples of a fundamental frequency) which had a wavelength directly related to the length of the femoral canal. This contrasts with initial strikes, where multiple unrelated frequencies were present.

Background. Joint line, patellar height and posterior condylar offset (PCO) are related to functional outcome such as stability and range of motion after revision total knee arthroplasty (TKA). The purpose of this study was (1) to determine whether revision TKA can restore the joint line, patella height and condylar offset after surgery, and (2) to assess factors associated with failed restoration. Materials and Methods. We retrospectively reviewed 27 consecutive patients who underwent revision TKA. Among 27 patients, 11 patients had two-stage revision TKA due to periprosthetic joint infection while 14 patients underwent revision TKA due to aseptic loosening. In addition, there were 2 patients who had traumatic event causing a periprosthetic fracture which led to revision TKA. The joint line was measured using the distance from the adductor tubercle of the femur to the most distal portion of the medial femoral component on knee anterior-posterior radiographs. The long-term result of a total hip arthroplasty (THA) strongly depends on the correct component positioning of the acetabular cup and stem. To measure cup orientation out of a postoperative anteroposterior (AP) pelvic radiograph is highly inaccurate due to the wide variation of individual pelvic tilt and rotation. The goal of this study was to develop and validate a 2D-3D matching software (HipMatch) that allows matching a postoperative AP pelvic radiograph with a preoperative CT to accurately measure cup orientation corrected for individual pelvic orientation. The software is based on a spline-based multi-resolution 2D-3D image registration algorithm and a Markov random field theory based on similarity measurement. Based on a cone projection (imitating the path of the x-rays), the software is able to match the three-dimensional CT-based data set with the contours of the projected pelvis on the AP pelvic radiograph. This gives the possibility to correct the measured cup orientation (inclination and anteversion) by measuring it according to an anatomical defined coordinate system (anterior pelvic plane). The validation of the software consisted of accuracy, reproducibility and observer reliability measurements using cadaver and clinical data. For the cadaver validation 10 human pelves (20 hips) were used. From each pelvis 2 CT scans, one with and one without an inserted cup were acquired. The CT scan with the cup was used as the ground truth. With the cup inserted 4 AP pelvic radiographs with the pelvis in an unknown arbitrary position during acquisition were performed resulting in 80 measurements for accuracy. These measurements were performed by 2 observers at 2 different occasions resulting in a total of 320 measurements for reproducibility and observer reliability. The intraclass correlation coefficient (ICC) was used for quantification of reproducibility and observer reliability and the Bland-Altman analysis was used to detect systemic errors.
