The aims of treatment are the same, including 'getting it right first time' with a single operation, which allows immediate unrestricted weightbearing, with a low risk of complications, and one that avoids the creation of stress risers locally that may predispose to further peri-implant fracture. Biological reconstruction techniques after diaphyseal tumour resection have increased in popularity in recent years. High complication and failure rates have been reported with intercalary allografts, with recent studies questioning their role in limb-salvage surgery. We developed a technique in which large segment allografts are augmented with intramedullary cement and fixed using compression plating. The goal of this study was to evaluate the survivorship, complications and functional outcomes of these intercalary reconstructions. Forty-two patients who had reconstruction with an intercalary allograft following tumour resection between 1989 and 2010 were identified from our prospectively collected database. Allograft survival, local recurrence-free, disease-free and overall survival were assessed using the Kaplan-Meier method. Patient function was assessed using the Musculoskeletal Tumour Society (MSTS) scoring system and the Toronto Extremity Salvage Score (TESS). The 23 women and 19 men had a mean age of 33 years (14–77). The most common diagnoses were osteosarcoma (n=16) and chondrosarcoma (n=9). There were 9 humerus, 18 femur and 15 tibia reconstructions. At a mean follow-up of 95 months (5–288), 31 patients were alive without disease, 10 were dead of disease and 1 was deceased of other causes. There were 4 local recurrences and 11 patients developed metastatic disease. 5-year local recurrence free survival was 92%, 5-year disease-free survival was 70% and overall survival was 75%. Fourteen of 42 patients (33%) experienced complications: 5 wound healing complications, 4 infections, 2 non-unions, 2 fractures and 1 nerve palsy. Four allografts (9.5%) were revised for complications and 2 (5%) for local recurrence. Mean allograft survival was 85 months (4–288). Mean time to union was 8.2 (3–36) months for the proximal osteotomy site and 8.1 (3–23) months for the distal osteotomy site. Aims. The incidence of atypical femoral fractures (AFFs) continues to increase. However, there are currently few long-term studies on the complications of AFFs and factors affecting them. Therefore, we attempted to investigate the outcomes, complications, and risk factors for complication through mid-term follow-up of more than three years. Methods. From January 2003 to January 2016, 305 patients who underwent surgery for AFFs at six hospitals were enrolled. After exclusion, a total of 147 patients were included with a mean age of 71.6 years (48 to 89) and 146 of whom were female. We retrospectively evaluated medical records, and reviewed radiographs to investigate the fracture site, femur bowing angle, presence of delayed union or nonunion, contralateral AFFs, and peri-implant fracture. A statistical analysis was performed to identify the significance of associated factors. Results. The mean follow-up period was 70.2 months (36 to 191). There were 146 AFFs (99.3%) in female patients and the mean age was 71.6 years (48 to 89). The AFFs were located in the subtrochanter and shaft in 52 cases (35.4%) and 95 (64.6%), respectively. The preoperative mean anterior/lateral femoral bowing angles were 10.5° (SD 5.7°)/6.1° (SD 6.2°). The postoperative mean anterior/lateral bowing values were changed by 8.7° (SD 5.4°)/4.6° (SD 5.9°). Bisphosphonates had been used contemporarily in 115 AFFs (78.2%) for a mean of 52.4 months (1 to 204; SD 45.5) preoperatively. Nailing was performed in 133 AFFs (90.5%), and union was obtained at a mean of 23.6 weeks (7 to 85). Delayed union occurred in 41 (27.9%), and nonunion occurred in 13 (8.8%). Contralateral AFF occurred in 79 patients (53.7%), and the use of a bisphosphonate significantly influenced the occurrence of contralateral AFFs (p = 0.019). Peri-implant fractures occurred in a total of 13 patients (8.8%), and a significant increase was observed in cases with plating (p = 0.021) and high grade of postoperative anterolateral bowing (p = 0.044). There is controversy regarding the optimum method of stabilising traumatic anterior pelvic ring injuries. This study aimed to evaluate the role of pubic symphysis plating. Methods. All patients who underwent pubic symphysis plating in a regional pelvic and acetabular unit were studied. Fracture classification, type of fixation, complications, and incidence of metalwork failure were recorded. Results. Out of 178 consecutive patients, 159 (89%) were studied for a mean of 37.6 months. There were 121 males and 38 females (mean age 43 years). Symphysis pubic fixation was performed in 100 AO-OTA type B and 59 type C injuries using a Matta symphyseal plate (n=92), reconstruction plate (n=65), or DCP (n=2). Supplementary posterior pelvic fixation was performed in 102 patients. 5 patients required revision for failure of fixation or symptomatic instability of the pubic symphysis. A further 7 patients had metalwork removed for other reasons. Metalwork breakage occurred in 63 patients (40%). 62 of these 63 patients were asymptomatic and metalwork was left in situ. Conclusions.

Introduction. To compare the union rates and post-operative mobility of antegrade intramedullary nailing of osteoporotic traumatic supracondylar femoral fractures (AO classification A to C2) with those of plating. Materials/Methods. We studied any traumatic intra or extra-articular supracondylar femoral fracture from 2005–2010. Patients were either admitted directly to our level 1 trauma centre or were referred from another hospital. Nineteen patients were identified, consisting of primarily fixation with five antegrade nails and fourteen plates. We defined osteoporotic bone as being present in anyone over sixty years old or who had a clinical diagnosis. One nail and six plates were excluded due to young age or fracture severity. This left four nails, six less invasive stabilisation system plates and two dynamic condylar screw plates. Both groups were comparable with respect to age, sex and AO fracture classification. Results. There was a significant difference in achieving union between the two groups (p=0.040). Union occurred within three months in all four fractures in the nail group but only three fractures (38%) united after primary fixation in the plate group. There were two failures due to screw pullout, one failure due to screw breakage, one broken plate after delayed-union and one screw breakage after non-union. The patients in the nail group had better mobility and less pain than the plate group but the difference was not statistically significant. Conclusion. Materials and methods: Between November 2002 and April 2003, 23 patients (15 women, 8 men), mean age 55, (17–80) underwent open reduction and internal fixation using 3.5 mm locking compression oblique T plate (SYNTHES) through a Henry ‘s approach and a 2 weeks plaster cast immobilisation. All fractures were dorsally displaced. According to AO classification there were 15 A3 and 8 C1 and C2 fracture. 18 patients could be reviewed with a mean follow up of 16 months (6 to 30) Pre operatively, radial inclination was 11.7 ° (0–20), dorsal angulation 25.9 ° (8–48) and ulnar variance:4 mm (0–10). Results: Post-operatively, radial inclination was 23,2. ulnar variance: 1,2 mm and ventral angulation 4,6 °. At one year follow-up, there was no loss of post-operative reduction. According to SOFCOT ‘s criteria, there were 13 anatomical results and 5 moderate malunion. According to Green and O ‘Brien’s criteria, there were 9 excellent, 6 good, 3 fair and no poor results. The Proximal Humeral Internal Locking System (PHILOS) is being used increasingly in the treatment of proximal humeral fractures. Improvements in operative technique since its first use may represent improved functional results. We analysed 28 cases performed in the two years following October 2006. There were 22 females and 6 males. Mean age was 65 years (Range 37–79 years). There were five 2-part, nineteen 3-part, and four 4-part fractures. Functional results were measured using the Oxford Shoulder Score, American Shoulder and Elbow Surgeons’ Score (ASES) and Constant Score (Age and sex matched). This provided objective and subjective scores of function after a mean follow-up period of 15 months (Range 3–27 months). These results were compared to the patient’s non-operated shoulder to determine loss in function. Following PHILOS fixation, function of the operated arm was decreased both subjective and objective scoring. This change was significant for all three scores (p-values <

The rigid fixation of glenoid base plate is essential for the prevention of dissociation of the construct in the reverse total shoulder arthroplasty. For the rigid fixation, ideal placement of fixation screw is crucial but it is difficult to determine the best direction and length of screws. The purpose of this study was to determine configuration of optimal screw in cadaveric scapulae and compare with that in patient who underwent reverse total shoulder arthroplasty. Seven scapulae were used and implanted using a variable angle base plate with four directions screws. Optimal screw placement was defined as that which maximized screw length, accomplished far cortical purchase. Insertion angle and length of every screw was measured from AP and axial radiograph taken after the screws fixation. In a similar manner, the insertion angles of screws were measured from radiographs of 7 postoperative patients who underwent reverse total shoulder arthroplasty. This edition of Cochrane Corner looks at some of the work published by the Cochrane Collaboration, covering interventions for treating distal femur fractures in adults; ultrasound and shockwave therapy for acute fractures in adults; and local corticosteroid injection versus placebo for carpal tunnel syndrome.
The Proximal Humeral Internal Locking System (PHILOS) is being used increasingly in the treatment of proximal humeral fractures. Improvements in operative technique since its first use may represent improved functional results. We analysed 28 cases performed in the two years following October 2006. There were 22 females and 6 males. Mean age was 65 years (Range 37–79 years). There were five 2-part, nineteen 3-part, and four 4-part fractures. Functional results were measured using the Oxford Shoulder Score, American Shoulder and Elbow Surgeons’ Score (ASES) and Constant Score (Age and sex matched). These results were compared to the patient’s non-operated shoulder to determine loss in function. Following PHILOS fixation, the Oxford Score increased by a mean of 11 points, the ASES decreased by a mean of 25 points, and the Constant Score decreased by a mean of 31. This change was significant for all three scores (p-values <
0.01). Sub-analysis comparing patients aged <
60 years with those >
60 years and 2-part fractures with 3- and 4-part fractures showed no significant difference. We conclude that our results confirm previously published functional data and show the patient’s age, and fracture configuration, have no impact on the results of this implant.
The design and use of a prong plate for the treatment of displaced fractures of the femoral neck is described. The prongs hold the femoral head, and the plate is screwed to the anterior cortex of the femoral neck and trochanteric region. The new plate has been used in 32 patients with Garden stage III or IV fractures and 29 had a good result. Bony union was seen within 11 weeks in all patients in whom bleeding of the femoral head was noted at operation, but was much delayed in patients without bleeding. Late segmental aseptic necrosis was seen in four patients after follow-up of at least two years, and successfully treated. The prong plate allows accurate reduction, impaction and strong rigid fixation with less disturbance of the intramedullary blood flow than other fixation methods.
The PHILOS plate attempts to improve on previously designed implants by preserving the biological integrity of the humeral head articular fragment. The minimally invasive technique minimises soft tissue damage whilst at the same time securing the reduction using multiple locking screws with angular stability, thus allowing early mobilisation.
We also present a cost analysis relating to the use of the PHILOS plate in practice.
Mean OSS and DASH at follow up were 24.8 (S.D 11.6) and 28.0 (S.D 26.9) respectively. Worse scores were seen in those patients who had complications (OSS 39.4 Vs 22.6, DASH 58.2 Vs 23.4) although this difference was not statistically significant. There was also a trend for poorer scores with increasing fracture complexity and better scores in those patients operated upon by surgeons with a specific interest in the upper limb and in patients whose surgeon had performed more than 5 fixations.
1. A simple method of internal fixation of adult forearm fractures which gives consistent good results is necessary because the closed method of treatment is of limited application. 2. Open reduction of fractures without rigid internal fixation gives a high proportion of non-union and poor results. 3. Rigid internal fixation with standard plates and screws has been shown to give a low incidence of non-union in this series. 4. A three and a half inch long plate with six screws is suitable for most fractures, but if there is moderate or severe comminution, or if there is a segmental fracture, longer plates and more screws should be used to provide sound fixation. 5. Severely comminuted fractures with large avascular bone fragments should have the addition of a bone graft at the time of the plating operation in order that union may be assured. Thin strips of iliac bone are preferred. 6. Rigid plating is considered to be the most satisfactory treatment for open fractures. 7. Immobilisation of the limb after operation is not necessary and is undesirable if the fixation is rigid. 8. The functional results of this treatment are good and seemingly better than those achieved by other methods. 9. Serious complications of the plating operation are few and avoidable.
Clavicular hook plates have been used over the last decade in the treatment of lateral clavicular fractures with good rates of union reported throughout the literature. Fewer studies have reported the functional outcome of these patients and some have reported potential soft tissue damage post plate removal. We aimed to review the functional outcomes alongside union rates in patients treated with hook plates for lateral clavicular fractures. In this retrospective case series, 21 patients with traumatic lateral third clavicular fractures were included. 15 had Neer type II fractures, 4 Neer type III fractures, 1 patient had a Neer type I fracture and 1 radiograph was not able to be classified. All patients were treated with clavicular hook plates at the same district general hospital by five experienced surgeons between March 2010 and February 2015 adhering to the same surgical protocol. All patients had standard physiotherapy and post operative follow up. Plates were removed when radiological union was achieved in all but one patient who had the plate removed before union was achieved due to prolonged non-union. Patients were followed up post plate removal and evaluated clinically using the Oxford Shoulder Score. Their post plate radiographs were assessed by an independent radiologist and bony union documented. 21 patients were included. Mean age was 40 (range 14–63) with a male:female ratio of 17:4. Mean follow up was 5 months post injury (1–26 months). The hook plate remained in situ for a mean time of 4.3 months (2–16 months). One patient developed a post-operative wound infection treated with antibiotics, 2 patients developed adhesive capsulitis, one patient had not achieved bony union prior to hook plate removal at 16 months, however did achieve union 2 months post plate removal, two patients required revision plating. All patients achieved bony union eventually with good alignment and no displacement of the acromioclavicular joint seen on the most recent post operative radiographs. Post plate removal Oxford Shoulder Scores indicated good shoulder function with a mean score of 41.5 (maximum score possible 48 and the range of scores for our cohort was 30–47). Our data would support the use of hook plates in the treatment of lateral clavicular fractures. All patients achieved union eventually with good alignment and this was reflected in the good functional outcome scores. This study is limited in its small cohort and short-term follow up. More research is required to examine the long term consequences of hook plate surgery in a larger patient population.
High tibial osteotomy generally helps patients to postpone the TKA or even stay in peace for rest of their life, but sometimes these procedures enhance the process of osteoarthritis (1) several reasons like unhealthy lateral compartment of the knee or age or weight or concomitant debilitating diseases could be included in account. In this study we focused on those patients that were selected properly with correct procedure but still shows the knee OA enhancement. Hypothesis: probably bone bruise around the site of osteotomy as trauma of surgery can make a bad condition. As a RCT study between 2 groups with different technique of osteotomy especially the distance of osteotomy site from the joint line of proximal tibia was our main reason to choose the different procedures. We did a randomized clinical trial with not more than a year follow up and mostly emphasis in geometry of bone bruise around osteotomy site. Group A: 50 knees, open wedge and plate technique. Group B: reversed-v MIS the same 50 other knees (FM) Method: All patient asked for MRI before and in 1st 10 days post surgery from their target knees. Those with positive bone bruise sign before surgery were excluded. In post op MRIs we measured the geometry of bone bruise. 1- Our finding shows upward-downward length of bone bruise in MIS (F.M) reversed-v = 14–40 mm and in open wedge = 14–37 mm Depth (Medial-lateral) and AP diameter almost the same. 2- To omit the bias of bone bruise around the open wedge technique we ignored this part. Result and conclusion: Group A: had 14 – 40 mm bone bruise that in 61% reached to sub chondral bone (distance of osteotomy's site from sub chondral bone). Area was (17−4mm). Group B: because of the distance of osteotomy site from joint line were 60–70 mm. in no one bone bruise was closer than 26mm to sub chondral bone. So, our Iatrogenic bone bruise from joint line in reversed - v is in safer zone than open wedge and plating with p. value of 0.0001. In future we need to follow our patients to be sure if bone bruise makes any hazard for the knees.
Massive cemented endoprosthesis are used to enable early resumption of activity after tumour surgery. The longevity of the prosthesis varies with anatomical site, pros-thesis type, and mode of þxation. Revision surgery will be required in approximately 50% of cases of endopros-thetic replacements around the knee by 10 years because of aseptic loosening. Insertion of a second cemented endoprosthesis is a challenge because of the poor quality of the remaining bone and loosening recurs quickly. The use of extracortical plate þxation in joint sparing surgery where the remaining bone after tumour resection will not accept an intramedullary stem is also described. The þrst series of 14 patients with extracortical plate þxation in difþcult revision or joint-sparing tumour surgery with a mean follow-up of 5 years are described. The three-plate design incorporates well within a remodelled cortex to achieve osseomechanical integration with all patients regaining their premorbid level of function within 5 months. At 5 years the Enneking scores averaged 27.3. One revision was required in a femoral replacement because of loosening. It was possible to insert a new endo-prosthesis as the intramedullary bone had reconstituted. The preliminary results suggest that this technique may provide an easy, biomechanically friendly alternative to a device with an intramedullary stem, which has a shorter lifespan in revision tumour surgery.
The overall fusion success rate in our series was 90%, while pseudoarthrosis occurred in 5 patients (10%), with screw breakage in 1 patient (2%). Two of these patients had bone graft supplementation and in other 2 patients was done anterior fusion.
This paper describes the preliminary results of a proximal tibial endoprosthesis which spares the knee joint and enables retention of the natural articulation by replacing part of the tibial metaphysis and diaphysis. In eight patients who had a primary malignant bone tumour of the proximal tibia, the distal stem, which had a hydroxyapatite-coated collar to improve fixation, was cemented into the medullary canal. The proximal end had hydroxyapatite-coated extracortical plates which were secured to the remaining proximal tibial metaphysis using cortical screws. The mean age of the patients at operation was 28.9 years (8 to 43) and the mean follow-up was for 35 months (4 to 48). The mean Musculoskeletal Tumour Society score was 79% (57% to 90%), the mean Oxford Knee score was 40 points of 48 (36 to 46) and the mean knee flexion was 112° (100° to 120°). In one patient, revision to a below-knee amputation through the prosthesis was required because of recurrence of the tumour. Another patient sustained a periprosthetic fracture which healed with a painful malunion. This was revised to a further endoprosthesis which replaced the knee. In the remaining six patients the prosthesis allowed preservation of the knee joint with good function and no early evidence of loosening. Further follow-up is required to assess the longevity of these prostheses.
Periprosthetic femoral fractures are a challenging problem to manage. In the literature various constructs have been designed and tested, most requiring cables for proximal fixation. The Synthes Locking Attachment Plate (LAP) has been designed to achieve proximal fixation without the use of cables. The aim of this study was to biomechanically evaluate the LAP construct in comparison to a Cable plate construct, for the fixation of periprosthetic femoral fractures after cemented total hip arthroplasty (THA). Twelve synthetic femora were tested in axial compression, lateral bending and torsion to determine initial stiffness, and stiffness following fixation of a simulated midshaft fracture with and without a bone gap. Two different fracture fixation constructs (six per group) were assessed. Each construct incorporated a broad curved LCP with bi-cortical locking screws for distal fixation. In the Cable construct, 2 cables and 2 uni-cortical locking screws were used for proximal fixation. In the LAP construct, the cables were replaced by a LAP with 4 bicortical locking screws. Axial, lateral bending and torsional stiffness were assessed using intact specimen values as a baseline. Axial load to failure was also measured. The LAP construct was significantly stiffer than the cable construct under axial load with a bone gap (simulating a comminuted fracture) (p=0.01). There were no significant differences between the two constructs in any of the other modalities tested. Loading to failure resulted in no significant differences between constructs, in either initial stiffness or peak load. In conclusion the LAP construct enables bi-cortical screw fixation around a prosthesis. Compared to cables, this was stiffer when there was a bone gap and thus should offer improved proximal fixation of Vancouver B1 proximal femoral fractures in cemented THA.
We evaluated the biomechanical properties of two different methods of fixation for unstable fractures of the proximal humerus. Biomechanical testing of the two groups, locking plate alone (LP), and locking plate with a fibular strut graft (LPSG), was performed using seven pairs of human cadaveric humeri. Cyclical loads between 10 N and 80 N at 5 Hz were applied for 1 000 000 cycles. Immediately after cycling, an increasing axial load was applied at a rate of displacement of 5 mm/min. The displacement of the construct, maximum failure load, stiffness and mode of failure were compared. The displacement was significantly less in the LPSG group than in the LP group (p = 0.031). All maximum failure loads and measures of stiffness in the LPSG group were significantly higher than those in the LP group (p = 0.024 and p = 0.035, respectively). In the LP group, varus collapse and plate bending were seen. In the LPSG group, the humeral head cut out and the fibular strut grafts fractured. No broken plates or screws were seen in either group. We conclude that strut graft augmentation significantly increases both the maximum failure load and the initial stiffness of this construct compared with a locking plate alone.
The purpose of this study is to describe the use of the PHILOS plate (Synthes) in reverse configuration to treat complex distal humeral non-unions. Non-union is a frequent complication of distal humeral fracture. It is a challenging problem due to the complex anatomy of the distal humerus, small distal fragment heavily loaded by the forearm acting as a long lever arm with powerful forces increasing the chances of displacement. Rigid fixation and stability with a device of high “pull-out” strength is required. The PHILOS plate has been used in reverse configuration to achieve good fixation while allowing central posterior placement of the implant. 11 patients with established non-union of distal humeral fractures were included in this study. No patient in whom this implant was used has been excluded. Initial fixation was revised using the PHILOS plate in reverse configuration and good fixation was achieved. Bone graft substitutes were used in all cases. Patients were followed to bony union, and functional recovery. All fractures united. One required revision of plate due to fatigue failure. Average time to union was 8 months with excellent restoration of elbow function. A reversed PHILOS plate provides an excellent method of fixation in distal humeral non-union, often complicated by distorted anatomy and previous surgical intervention. It has a high “pull-out” strength and may be placed in the centre of the posterior humerus, allowing proximal extension of the fixation as far as is required. It provides secure distal fixation without impinging on the olecranon fossa. It is more versatile and easier to use than available pre contoured plating systems.
The þnal outcome, using the Modiþed Mayo Wrist Score was excellent in 13 cases, good in 7, and fair in 4 cases. Complications were seen in 3 patients and metalwork has subsequently been removed in 7 patients for tendon irritation but there have been no cases of extensor tendon rupture.
The purpose of this study was to evaluate the
risk of late displacement after the treatment of distal radial fractures with
a locking volar plate, and to investigate the clinical and radiological
factors that might correlate with re-displacement. From March 2007
to October 2009, 120 of an original cohort of 132 female patients
with unstable fractures of the distal radius were treated with a
volar locking plate, and were studied over a follow-up period of
six months. In the immediate post-operative and final follow-up
radiographs, late displacement was evaluated as judged by ulnar
variance, radial inclination, and dorsal angulation. We also analysed
the correlation of a variety of clinical and radiological factors
with re-displacement. Ulnar variance was significantly overcorrected
(p <
0.001) while radial inclination and dorsal angulation were
undercorrected when compared statistically (p <
0.001) with the unaffected
side in the immediate post-operative stage. During follow-up, radial
shortening and dorsal angulation progressed statistically, but none
had a value beyond the acceptable range. Bone mineral density measured
at the proximal femur and the position of the screws in the subchondral
region, correlated with slight progressive radial shortening, which
was not clinically relevant. Volar locking plating of distal radial fractures is a reliable
form of treatment without substantial late displacement. Cite this article:
Volar Locking Plates (VLP) have revolutionised the treatment of distal radius fractures allowing the anatomic reduction and stable fixation of the more comminuted and unstable of fractures. The benefits of this in terms of range of movement (ROM), pain and earlier return to work and daily activities is documented. However we were interested in was what improvements in wrist function patients made from 6 to 12 months after injury? We retrospectively looked at a series of 34 consecutive patients that had undergone VLP fixation through a standard anterior approach followed by early physiotherapy. We documented standard demographics and assessed function in terms of Range of Movement, Grip strength (GS), Modified Gartland and Werley score (MGWS), Patient Rated Wrist Evaluation (PRWE) and the quick DASH questionnaire at six and twelve months Two patients were excluded from analysis as they failed to make both assessments. Of the 32 remaining (26 female:6 male) the mean age was 53.2yrs; range (26–78). On average GS, PGS, VAS function and pain did not improve. There was a modest improvement in Movement; Wrist Flexon-13 deg, Wrist Extension-14deg, Radial Deviation-7deg, Ulnar Deviation-9deg. There was no improvement in pronation and supination. There was little improvement in qDASH, PRWE and mGW Scores with only a mean 1.8, 5.6 and 3.6 point improvement respectively.Methods
Results
In June 1999 we instituted a prospective, randomised, double blind study to determine whether in the use of the Profix® total knee replacement system the addition of screws improved the fixation of the tibial base plate. There were 145 arthroplasties in 137 patients, 77% of them female. Their mean age was 66 years. In 86% of the patients there was osteoarthritis and in 14% an inflammatory arthritis. On a random basis, supplementary screws were inserted through the base plate into the tibia in 49% of arthroplasties and the remaining 51% were not fixed. In both groups the postoperative femoral angle measured 95% and at 12-month evaluation the tibial angle measured 89°, giving a total valgus angle of 6°. To date no tibial base plates have come loose. It is felt that the large central titanium peg plays a major role in assisting fixation of the base plate to the tibia and that supplementary screws are unnecessary. There is a small cost saving when screws are not used. We postulate that one may in the future minimise polyethylene debris tracking and osteolysis.
