Aims. The aim of this study was to assess the clinical and radiological outcomes of an antiprotrusio acetabular cage (APC) when used in the surgical treatment of periacetabular bone metastases. Methods. This retrospective cohort study using a prospectively collected database involved 56 patients who underwent acetabular reconstruction for periacetabular bone metastases or haematological malignancy using a single APC between January 2009 and 2020. The mean follow-up was 20 months (1 to 143). The primary outcome measure was implant survival. Postoperative radiographs were analyzed for loosening and failure. Patient and implant survival were assessed using a competing risk analysis. Secondary parameters included primary malignancy, oncological treatment, surgical factors, length of stay in hospital, and postoperative complications. Results. A total of 33 patients (59%) died during the study period at a mean of 15 months postoperatively (1 to 63). No patient had radiological evidence of loosening or failure. Acetabular component survival was 100%. Three patients (5.4%) had further surgery; one (1.8%) underwent revision of the femoral component for dislocation, one required debridement with implant retention for periprosthetic joint infection, and one required closed reduction for dislocation. Using death as a competing risk, at 100 months, the probability of revision was 0.036 and the risk of death was 0.84. Conclusion. With appropriate patient selection, the antiprotrusio cage offers good implant survival, with a reasonable perioperative complication rate in this high-risk group of patients when managing metastatic disease or haematological malignancy around the acetabulum. Aims. Although periacetabular osteotomies are widely used for the treatment of symptomatic dysplastic hips, long-term surgical outcomes and patient-reported outcome measures (PROMs) are still unclear. Accordingly, we assessed hip survival and PROMs at 20 years after transpositional osteotomy of the acetabulum (TOA). Methods. A total of 172 hips in 159 patients who underwent TOA were followed up at a mean of 21.02 years (16.6 to 24.6) postoperatively. Kaplan-Meier analysis was used to assess survivorship with an endpoint of total hip arthroplasty (THA). PROMs included the visual analogue scale (VAS) Satisfaction, VAS Pain, Oxford Hip Score (OHS), and Forgotten Joint Score-12 (FJS-12). Thresholds for favourable outcomes for OHS (≥ 42) and FJS-12 (≥ 51) were obtained using the receiver operating characteristic curve with VAS Satisfaction ≥ 50 and VAS Pain < 20 as anchors. Results. THA was performed on 37 hips (21.5%) by the latest follow-up. Kaplan-Meier analysis indicated that the hip survival rate at 20 years was 79.7% (95% confidence interval (CI) 73.7 to 86.3). Multivariate analysis showed that preoperative Tönnis grade significantly influenced hip survival. Tönnis grades 0, 1, and 2 were associated with 20-year survival rates of 93.3% (95% CI 84.8 to 100), 86.7% (95% CI 79.8 to 94.3), and 54.8% (95% CI 41.5 to 72.3), respectively. More than 60% of the patients exhibited favourable PROMs. An advanced Tönnis grade at the latest follow-up and a higher BMI were both significantly associated with unfavourable OHS, but not with other PROMs. Conclusion. This study demonstrated the durability of TOA for hips with Tönnis grades 0 to 1 at 20 years. While the presence of advanced osteoarthritis and higher BMI was associated with lower hip functions (OHS), it was not necessarily associated with worse patient satisfaction and joint awareness. Aims. The aim of this study was to determine the clinical outcomes and factors contributing to failure of transposition osteotomy of the acetabulum (TOA), a type of spherical periacetabular osteotomy, for advanced osteoarthritis secondary to hip dysplasia. Methods. We reviewed patients with Tönnis grade 2 osteoarthritis secondary to hip dysplasia who underwent TOA between November 1998 and December 2019. Patient demographic details, osteotomy-related complications, and the modified Harris Hip Score (mHHS) were obtained via medical notes review. Radiological indicators of hip dysplasia were assessed using preoperative and postoperative radiographs. The cumulative probability of TOA failure (progression to Tönnis grade 3 or conversion to total hip arthroplasty) was estimated using the Kaplan-Meier product-limited method. A multivariate Cox proportional hazards model was used to identify predictors of failure. Results. This study included 127 patients (137 hips). Median follow-up period was ten years (IQR 6 to 15). The median mHHS improved from 59 (IQR 52 to 70) preoperatively to 90 (IQR 73 to 96) at the latest follow-up (p < 0.001). The survival rate was 90% (95% CI 82 to 95) at ten years, decreasing to 21% (95% CI 7 to 48) at 20 years. Fair joint congruity on preoperative hip abduction radiographs and a decreased postoperative anterior wall index (AWI) were identified as independent risk factors for failure. The survival rate for the 42 hips with good preoperative joint congruity and a postoperative AWI ≥ 0.30 was 100% at ten years, and remained at 83% (95% CI 38 to 98) at 20 years. Conclusion. Although the overall clinical outcomes of TOA in patients with advanced osteoarthritis are suboptimal, favourable results can be achieved in selected cases with good preoperative joint congruity and adequate postoperative anterior acetabular coverage. Acetabulum fractures caused by civilian firearms represent a unique challenge for orthopaedic surgeons. Treatment strategies should include the assessment of infection risk due to frequently associated abdominal injuries and maintenance of joint function. Still, internationally accepted treatment algorithms are not available. The aim of the study was to increase knowledge about civilian gunshot fractures of the acetabulum by describing their characteristics and management at a high-volume tertiary hospital. All adult patients admitted to our hospital between January 2009 and December 2022 with civilian gunshot fractures of the acetabulum were included in this descriptive retrospective study. In total our institution treated 301 patients with civilian gunshot fractures of the hip joint and pelvis during the observation period, of which 54 involved the acetabulum. Most patients were young males (88,9%) with a mean age of 29 years. Thirty patients (55,6%) had associated intraabdominal or urological injuries. Fracture patterns were mostly stable fractures with minor joint destruction amenable to conservative fracture treatment (n=48, 88,9%). Orthopaedic surgical interventions were performed in 21 patients (38,9%) with removal of bullets in contact with the hip joint via arthrotomy or surgical hip dislocation as most frequent procedures. Most patients received antibiotics on admission (n=49, 90,7%). Fracture related infections of the acetabulum were noted in six patients (11,1%) while the mortality in the study population was low with one demised patient (1,9%) due to the trauma burden. Most civilian acetabulum gunshot fractures are associated with intraabdominal or urological injuries. In comparison to the literature on extremity gunshot fractures, there is an increased risk of infection in our study population. The decision for surgical wash-out and bullet removal should be based on contamination and anticipated joint destruction, while osteosynthesis or primary arthroplasty are rarely necessary for these injuries

Aims. Surgical approaches that claim to be minimally invasive, such as the direct anterior approach (DAA), are reported to have a clinical advantage, but are technically challenging and may create more injury to the soft-tissues during joint exposure. Our aim was to quantify the effect of soft-tissue releases on the joint torque and femoral mobility during joint exposure for hip resurfacing performed via the DAA. Methods. Nine fresh-frozen hip joints from five pelvis to mid-tibia cadaveric specimens were approached using the DAA. A custom fixture consisting of a six-axis force/torque sensor and motion sensor was attached to tibial diaphysis to measure manually applied torques and joint angles by the surgeon. Following dislocation, the torques generated to visualize the acetabulum and proximal femur were assessed after sequential release of the joint capsule and short external rotators. Results. Following initial exposure, the ischiofemoral ligament (7 to 8 o’clock) was the largest restrictor of exposure of the acetabulum, contributing to a mean 25% of overall external rotational restraint. The ischiofemoral ligament (10 to 12 o’clock) was the largest restrictor of exposure of the proximal femur, contributing to 25% of overall extension restraint. Releasing the short external rotators had minimal contribution in torque generated during joint exposure (≤ 5%). Conclusion. Adequate exposure of both proximal femur and acetabulum may be achieved with minimal torque by performing a full proximal circumferential capsulotomy while preserving short external rotators. The joint torque generated and exposure achieved is dependent on patient factors; therefore, some cases may necessitate further releases. Aims. Limb salvage for pelvic sarcomas involving the acetabulum is a major surgical challenge. There remains no consensus about what is the optimum type of reconstruction after resection of the tumour. The aim of this study was to evaluate the surgical outcomes in these patients according to the methods of periacetabular reconstruction. Methods. The study involved a consecutive series of 122 patients with a periacetabular bone sarcoma who underwent limb-salvage surgery involving a custom-made prosthesis in 65 (53%), an ice-cream cone prosthesis in 21 (17%), an extracorporeal irradiated autograft in 18 (15%), and nonskeletal reconstruction in 18 (15%). Results. The rates of major complications necessitating further surgery were 62%, 24%, 56%, and 17% for custom-made prostheses, ice-cream cone prostheses, irradiated autografts and nonskeletal reconstructions, respectively (p = 0.001). The ten-year cumulative incidence of failure of the reconstruction was 19%, 9%, 33%, and 0%, respectively. The major cause of failure was deep infection (11%), followed by local recurrence (6%). The mean functional Musculoskeletal Tumour Society (MSTS) scores were 59%, 74%, 64%, and 72%, respectively. The scores were significantly lower in patients with major complications than in those without complications (mean 52% (SD 20%) vs 74% (SD 19%); p < 0.001). For periacetabular resections involving the ilium, the mean score was the highest with custom-made prostheses (82% (SD 10%)) in patients without any major complication; however, nonskeletal reconstruction resulted in the highest mean scores (78% (SD 12%)) in patients who had major complications. For periacetabular resections not involving the ilium, significantly higher mean scores were obtained with ice-cream cone prostheses (79% (SD 17%); p = 0.031). Conclusion. Functional outcome following periacetabular reconstruction is closely associated with the occurrence of complications requiring further surgery. For tumours treated with periacetabular and iliac resection, skeletal reconstruction may result in the best outcomes in the absence of complications, whereas nonskeletal reconstruction is a reasonable option if the risk of complications is high. For tumours requiring periacetabular resection without the ilium, reconstruction using an ice-cream cone prosthesis supported by antibiotic-laden cement is a reliable option. Aims. To clarify the mid-term results of transposition osteotomy of the acetabulum (TOA), a type of spherical periacetabular osteotomy, combined with structural allograft bone grafting for severe hip dysplasia. Methods. We reviewed patients with severe hip dysplasia, defined as Severin IVb or V (lateral centre-edge angle (LCEA) < 0°), who underwent TOA with a structural bone allograft between 1998 and 2019. A medical chart review was conducted to extract demographic data, complications related to the osteotomy, and modified Harris Hip Score (mHHS). Radiological parameters of hip dysplasia were measured on pre- and postoperative radiographs. The cumulative probability of TOA failure (progression to Tönnis grade 3 or conversion to total hip arthroplasty) was estimated using the Kaplan–Meier product-limited method, and a multivariate Cox proportional hazard model was used to identify predictors for failure. Results. A total of 64 patients (76 hips) were included in this study. The median follow-up period was ten years (interquartile range (IQR) five to 14). The median mHHS improved from 67 (IQR 56 to 80) preoperatively to 96 (IQR 85 to 97) at the latest follow-up (p < 0.001). The radiological parameters improved postoperatively (p < 0.001), with the resulting parameters falling within the normal range in 42% to 95% of hips. The survival rate was 95% at ten years and 80% at 15 years. Preoperative Tönnis grade 2 was an independent risk factor for TOA failure. Conclusion. Our findings suggest that TOA with structural bone allografting is a viable surgical option for correcting severely dysplastic acetabulum in adolescents and young adults without advanced osteoarthritis, with favourable mid-term outcomes. In late developmental dysplasia of the hip in childhood, the deformed dysplastic acetabulum is malaligned and has lost its shape due to pressure from the subluxed femoral head. The outer part of the acetabulum involves the upper part of the original acetabulum, thereby giving a bipartite appearance. A clear edge separates the outer from inner part which represents the lower part of the original acetabulum and has no direct contact with the femoral head. Combined pelvic osteotomy (CPO) using a Lance acetabuloplasty with either a Salter or a Pemberton procedure restores the original shape and realigns the acetabulum. A total of 20 children (22 hips), with a mean age of 46 months (28 to 94) at primary operation underwent CPO with follow-up for between 12 and 132 months. In each case concentric stable reduction with good acetabular cover was achieved and maintained throughout the period of follow-up

The bicompartmental acetabulum is one of the morphological changes which may be seen in children with Legg-Calvé-Perthes’ disease. Three-dimensional CT and MRI were used to analyse the detailed morphology of the acetabulum with special reference to its inner surface, in 16 patients with Perthes’ disease and a bicompartmental acetabulum. The bicompartmental appearance was seen on the coronal plane image through the acetabular fossa. The lunate surface was seen to grow laterally resulting in an increased mediolateral thickness of the triradiate cartilage. On the horizontal plane images, the acetabular fossa had deepened and had a distinct prominence at its posterior border. The combination of these morphological changes resulted in a bicompartmental appearance on plain radiography. Acetabular bicompartmentalisation appears to be the result of an imbalance of growth between the cartilage-covered lunate surface and the cartilage-devoid acetabular fossa

Aims. The aim of this study was to compare the biomechanical models of two frequently used techniques for reconstructing severe acetabular defects with pelvic discontinuity in revision total hip arthroplasty (THA) – the Trabecular Metal Acetabular Revision System (TMARS) and custom triflange acetabular components (CTACs) – using virtual modelling. Methods. Pre- and postoperative CT scans from ten patients who underwent revision with the TMARS for a Paprosky IIIB acetabular defect with pelvic discontinuity were retrospectively collated. Computer models of a CTAC implant were designed from the preoperative CT scans of these patients. Computer models of the TMARS reconstruction were segmented from postoperative CT scans using a semi-automated method. The amount of bone removed, the implant-bone apposition that was achieved, and the restoration of the centre of rotation of the hip were compared between all the actual TMARS and the virtual CTAC implants. Results. The median amount of bone removed for TMARS reconstructions was significantly greater than for CTAC implants (9.07 cm. 3. (interquartile range (IQR) 5.86 to 21.42) vs 1.16 cm. 3. (IQR 0.42 to 3.53) (p = 0.004). There was no significant difference between the median overall implant-bone apposition between TMARS reconstructions and CTAC implants (54.8 cm. 2. (IQR 28.2 to 82.3) vs 56.6 cm. 2. (IQR 40.6 to 69.7) (p = 0.683). However, there was significantly more implant-bone apposition within the residual acetabulum (45.2 cm. 2. (IQR 28.2 to 72.4) vs 25.5 cm. 2. (IQR 12.8 to 44.1) (p = 0.001) and conversely significantly less apposition with the outer cortex of the pelvis for TMARS implants compared with CTAC reconstructions (0 cm. 2. (IQR 0 to 13.1) vs 23.2 cm. 2. (IQR 16.4 to 30.6) (p = 0.009). The mean centre of rotation of the hip of TMARS reconstructions differed by a mean of 11.1 mm (3 to 28) compared with CTAC implants. Conclusion. In using TMARS, more bone is removed, thus achieving more implant-bone apposition within the residual acetabular bone. In CTAC implants, the amount of bone removed is minimal, while the implant-bone apposition is more evenly distributed between the residual acetabulum and the outer cortex of the pelvis. The differences suggest that these implants used to treat pelvic discontinuity might achieve short- and long-term stability through different biomechanical mechanisms. Although periacetabular osteotomies are widely used for the treatment of symptomatic acetabular dysplasia, the surgical outcomes after long term follow-up are still limited. Thus, we assessed hip survival and patient-reported outcomes (PROMs) at 20 years after the transposition osteotomy of acetabulum (TOA). Among 260 hips in 238 patients treated with TOA, 172 hips in 160 patients were evaluated at average 20.8 years, excluding patients who died or lost to follow-up. Kaplan-Meier analysis was used to assess survivorship with an end-point of THA. PROMs were evaluated using the VAS satisfaction, VAS pain, Oxford hip score (OHS), and Forgotten joint score (FJS). The thresholds of favorable outcomes of FJS and OHS were obtained using the receiver-operating characteristic curve with VAS satisfaction ≥ 50 and VAS pain < 20 as anchors. Thirty-three hips (19.2%) underwent THA at average 13.3 years after TOA. Kaplan-Meier analysis revealed hip survival rate at 20 years was 79.7%. Multivariate analysis showed the preoperative Tönnis grade significantly influenced hip survival. Survival rates with Tönnis grade 0, grade 1, and grade 2 were 93.3%, 86.7%, and 54.8% at 20 years, respectively. More than 60% of the patients showed favorable PROMs (VAS satisfaction ≥ 50, VAS pain < 20, OHS ≥ 42, FJS ≥ 51). Advanced Tönnis grade at the latest follow-up and higher BMI were significantly associated with unfavorable OHS, but not with other PROMs. This study demonstrated the durability of TOA for hips with Tönnis grade 0–1 and favorable satisfaction in majority of the patients at 20 years after surgery. Current presence of advanced osteoarthritis is associated with the lower hip function (OHS), but not necessarily associated with subjective pain and satisfaction. Higher BMI also showed a negative impact on postoperative function

Aims. Accumulated evidence indicates that local cell origins may ingrain differences in the phenotypic activity of human osteoblasts. We hypothesized that these differences may also exist in osteoblasts harvested from the same bone type at periarticular sites, including those adjacent to the fixation sites for total joint implant components. Methods. Human osteoblasts were obtained from the acetabulum and femoral neck of seven patients undergoing total hip arthroplasty (THA) and from the femoral and tibial cuts of six patients undergoing total knee arthroplasty (TKA). Osteoblasts were extracted from the usually discarded bone via enzyme digestion, characterized by flow cytometry, and cultured to passage three before measurement of metabolic activity, collagen production, alkaline phosphatase (ALP) expression, and mineralization. Results. Osteoblasts from the acetabulum showed lower proliferation (p = 0.034), cumulative collagen release (p < 0.001), and ALP expression (p = 0.009), and produced less mineral (p = 0.006) than those from the femoral neck. Osteoblasts from the tibia produced significantly less collagen (p = 0.021) and showed lower ALP expression than those from the distal femur. Conclusion. We have demonstrated for the first time an anatomical regional variation in the biological behaviours of osteoblasts on either side of the hip and knee joint. The lower osteoblast proliferation, matrix production, and mineralization from the acetabulum compared to those from the proximal femur may be reflected in differences in bone formation and implant fixation at these sites. Background. CT-based navigation system in total hip arthroplasty(THA) is widely used to achieve accurate implant placement. The purpose of this study was to evaluate the influence of initial error correction according to the differences in the shape of the acetabulum, and correction accuracy associated with operation approach after localization of registration points at anterior or posterior area of the acetabulum. Methods. We set the anterior pelvic plane(APP) as the reference plane, and defined the coordinates as follows: X-axis for external direction, Y-axis for anterior direction, and Z-axis for proximal direction. APP is defined by the anterior superior iliac spines and anterior border of the pubic symphysis. We made a bone model of bilateral acetabular dysplasia of the hip, after rotational acetabulum osteotomy(RAO) on one side, and performed registration using infrared-reflective markers. At first, we registered the initial error on navigation system, and calculated the accuracy of the error correction based on each shape of the acetabulum as we increased the surface matching points. Based on the actual operation approach, we also examined the accuracy of the error correction when concentrating the matching points in anterior or posterior areas of the acetabulum. Results. For the rotational acetabular osteotomy model, the range of possible initial error correction increased as the surface matching points increased on both X-axis and Y-axis: On the X-axis, the range increased from 6mm to 10mm as the surface matching point increased from 10 to 20; and on the Y-axis, the range increased from 2mm to 10mm as the point increased 10 to 50. The range did not increase on the Z-axis. For the acetabular dysplasia model, the range of possible initial error correction increased on the X-axis(the range increased from 2mm to 8mm as the point increased from 10 to 50); however, no increase was observed for the Y- and Z-axis. Furthermore, concentrating the surface matching points in the posterior area around the acetabulum was more effective for the correction of the initial rotational error. Discussion. Because of the different anatomical shapes of the acetabulum, the error directions that were difficult to correct tended to vary between dysplasia and post-RAO. The error correction of Z-axis was difficult on both shapes of the acetabulum. Thus, the careful initial setting on Z-axis is important to minimize the error. Surface matching point on the posterior part of the acetabulum is more effective in correcting the initial rotational error compared with the anterior part of the acetabulum. It was shown that the difference in the error correction was affected by the localization of the registration points around the acetabulum. We presumed that using surface matching points on posterior area of the acetabulum improves the accuracy of the CT-based navigation system on the anterior approach. When using the system, it is important to understand the tendency that the shape of the acetabulum and the localization of the surface matching points have influence on correction of the initial error

Introduction. The success of total hip replacement (THR) is closely linked to the positioning of the acetabular component. Malalignment increases rates of dislocation, impingement, acetabular migration, pelvic osteolysis, leg length discrepancy and polyethylene wear. Many surgeons orientate the cup to inherent anatomy of the acetabulum. Detailed understanding of the anatomy and orientation of the acetabulum in arthritic hips is therefore very important. The aim of this study was to describe the anteversion and inclination of the inherent acetabulum in arthritic hips and to identify the number that fall out with the ‘safe zone’ of acetabular position described by Lewinnek et al. (anteversion 15°±10°; inclination 40°±10°). Materials and Methods. A series of 65 hips all with symptomatic osteoarthritis undergoing THR were investigated. Patients with dysplastic hips were excluded. All patients had a navigated THR as part of their normal clinical treatment. A commercially available non image based computer navigation system (Orthopilot BBraun Aesculap, Tuttlingen, Germany) was used. Anterior pelvic plane was registered using the two anterior superior iliac spines and pubic symphysis. Inner size of the empty acetabulum was sized with cup trials and appropriately size trial fixed with a computer tracker was then aligned in the orientation of the natural acetabulum as defined by the acetabular rim ignoring any osteophytes. The inclination and anteversion were calculated by the software. The acetabular inclination in all hips was also measured on pre-operative anteroposterior pelvic digital radiographs. Acetabular inclination was measured using as the angle between a line passing through the superior and inferior rim of the acetabulum and a line parallel to the pelvis as identified by the tear drops, using the method described by Atkinson et al. Results. All patients were Caucasian and had primary osteoarthritis. There were 29 males and 36 females. The average age was 68 years (SD 8). The inclination was 50.4(SD 7.4) and 58.8(SD 5.7) on navigation and radiographs respectively. The anteversion was 9.3(SD 10.3) on navigation. Anteversion for males was significantly lower than females with a mean difference of −5.5° (95% CI −10.5°, −0.5°) with a p value of 0.033. There was no significant difference with respect to inclination. Overall 69% of patients had a combined inclination and anteversion of the native acetabulum that fell outside the “safe zone” of Lewinnek. Conclusions. Inherent acetabular orientation in arthritic hips falls out with the safe zone defined by Lewinnek in 69% of cases. When using the natural acetabular orientation as a guide for positioning implants it should therefore not be assumed this will fall with in the safe zone although the validity of safe zones itself is questionable. Variation between patients must be taken into account. The difference between males and females, particularly in terms of anteversion, should also be considered

The success of total hip replacement (THR) is closely linked to the positioning of the acetabular component. Malalignment increases rates of dislocation, impingement, acetabular migration, pelvic osteolysis, leg length discrepancy and polyethylene wear. Many surgeons orientate the cup in the same anteversion and inclination as the inherent anatomy of the acetabulum. The transverse acetabular ligament and acetabular rim can be used as a reference points for orientating the cup this way. Low rates of dislocation have been reported using this technique. Detailed understanding of the anatomy and orientation of the acetabulum in arthritic hips is therefore very important. The aim of this study was to describe the anteversion and inclination of the inherent acetabulum in arthritic hips and to identify the number that fall out with the ‘safe zone’ of acetabular position described by Lewinnek et al. (anteversion 15°±10°; inclination 40°±10°). A series of 65 hips, all with symptomatic osteoarthritis undergoing THR were investigated. Patients with developmental dysplastia of hip (DDH) were excluded. All patients had a navigated THR as part of their normal clinical treatment. A posterior approach to the hip was used. A commercially available non image based computer navigation system (Orthopilot BBraun Aesculap, Tuttlingen, Germany) was used. Rigid bodies (using active trackers) were attached to pelvis and femur. Anterior pelvic plane was registered using the two anterior superior iliac spines and pubic symphysis. The femoral head dislocated and removed and the labrum and soft tissue were excised to clear floor and rim of the acetabulum. Inner size of the empty acetabulum was sized with cup trials and appropriately size trial fixed with a computer tracker was then aligned in the orientation of the natural acetabulum as defined by the acetabular rim ignoring any osteophytes. The inclination and anteversion were calculated by the software. Surgery then proceeded with guidance of the computer navigation system. The computer software defines the anatomical values of orientation, to allow comparison with radiographs these were converted to radiological values as described by Murray et al. The acetabular inclination in all hips was also measured on pre-operative anteroposterior pelvic radiographs. This was done using digital radiographs analysed with the PACS system (Kodak, Carestream PACS Client, version 10.0). Acetabular inclination was measured using as the angle between a line passing through the superior and inferior rim of the acetabulum and a line parallel to the pelvis as identified by the tear drops, using the method described by Atkinson et al. All patients were Caucasian and had primary osteoarthritis. There were 29 males and 36 females. The average age was 68 years (SD 8). Mean anteversion was 9.3° (SD 10.3°). Anteversion for males was significantly lower than females with a mean difference of −5.5° (95%CI −10.5°,−0.5°) p = 0.033 but there was no significant difference in the number falling outside the “safe zone”. Thirty-four patients were studied through the whole of the Perthes’ disease in Alder Hey Children’s Hospital, Liverpool. The acetabular changes included osteopaenia of the roof, irregularity of its contour, and decrease in the depth. These changes were proportional to the femoral head involvement. The purpose of the study was to know the effect of the morphological changes of the femoral head on the acetabulum and its outcome. Radioisotope scans of the hip were examined in fourteen children with unilateral Perthes’ disease and comparison was made with the contralateral hip. These scans showed increased uptake on the lateral part of the acetabulum and no uptake over the avascular part of the femoral head. Average follow-up was ten years and on an average children were followed up from six years to fifteen years of age. Six readings of the measurements of various dimensions of the acetabulum and the femoral head were done. CT scan also showed irregularity in the acetabulum. Statistical tests lead to the conclusion that the decrease in the depth of the acetabulum was secondary to the femoral head involvement and the extent of its dimensional changes affected the final congruity between the femoral head and the acetabulum. Also the remodelling potential of acetabulum decreases as the child grows older. Therefore containment procedures could be done by femoral osteotomy in younger children and acetabular osteotomy may benefit older children

The purpose of this study is to determine how the lateral shape of the acetabulum changes during Perthes disease and if there is any correlation between the lateral acetabulum shape and final result and type of treatment. The study population consisted of 243 patients with unilateral involvement who had reached skeletal maturity at last follow up. There were 35 (14.4%) female and 208 (85.6%) male patients. The mean age at the onset of symptoms was 7 years and 1 month. AP X-ray films were estimated during fragmentation, reossification and last follow up. Group A consisted of 56 hips, 126 hips were classified as group B and 61 hips as group C according to the Herring classification. For the lateral acetabular shape we proposed a classification: group A – a normal concave acetabular roof, group B – a horizontal flat roof and group C – a roof convexly rounded and up going. All hips were treated by containment methods (bed rest and traction in abduction-78 hips, Petri cast-31 hips, brace-94 hips, varus osteotomy-20 hips, Salter oste-otomy-12 hips and shelf arthroplasty-8 hips). The outcomes of treatment were evaluated according to the Stulberg classification. During fragmentation stage we found 78 (32.1%) hips with normal lateral acetabular shape-type A. Horizontal roof-type B was noted in 136 (56%) hips and in 29 (11.9%) type C was observed. We observed improvement in the shape of lateral acetabulum after treatment. At the last follow up there were 124 (51%) hips with type A, 81 (33.3%) with type B and 38 (15.7%) with type C. Statistical analysis revealed significant correlation between lateral acetabular shape and Stulberg classification. A normal concave acetabular roof at the fragmentation stage leaded mainly to Stulberg group 1and 2 whereas a roof convexly rounded and up going leaded to Stulberg group 3, 4 or 5 (p< 0.0001). Analysis showed no statistical significant correlation between treatment by using bed rest and traction in abduction, Petri cast, braces and development the lateral acetab-ular shape (p=0.09). Only treatment by using surgical methods improved the lateral acetabular shape at the last follow up (p=0.0015). The acetabulum is a mould for remodeling of the deformed femoral head in Perthes disease and the lateral acetabulum plays the most important role. We can expect that normal shape of the acetabulum gives good result at final follow up whereas a roof convexly rounded usually follows to Stulberg group 3, 4 or 5. Only surgical treatment improves the shape of the acetabulum

Purpose of the study: Reconstruction of the acetabulum is becoming increasingly necessary due to the longer life expectancy of patients with a total hip arthroplasty (THA); it can also be needed after trauma or resection of a bone tumour. Material and methods: For 39 cases, we used a bone bank acetabulum with variable size for simple reconstruction of a part of the acetabulum (n=6) or to replace the entire acetabulum (n=19) or even an entire hemi-pelvis (n=14) in patients with more or less extensive bone loss. For 18 cases, resection was necessary to remove a bone tumour (16 chondrosarcomas and two giant-cell tumours), in 19 cases the reconstruction was necessary after multiple operations, and in four others due to traumatic destruction. Results: Allograft integration was successful in nearly all patients (two cases of necrosis required a secondary prosthesis, 12.5%) and in two cases we noted immunological reactions with a serous effusion. There were no local infections. For osteocartilaginous acetabuli (n=7) the integration was quite satisfactory, certain patients have been followed for more than 26 years with no evidence of osteoarthritis. Discussion: Use of a well-established protocol for hypothermia and the absence of secondary sterilisation yields grafts with preserved mechanical properties. The use of cyropreserved osteocartilaginous allografts offers hip for good integration free from degenerative osteoarthritis

Benefits of early stabilization of femoral shaft fractures, in mitigation of pulmonary and other complications, have been recognized over the past decades. Investigation into the appropriate level of resuscitation, and other measures of readiness for definitive fixation, versus a damage control strategy have been ongoing. These principles are now being applied to fractures of the thoracolumbar spine, pelvis, and acetabulum. Systems of trauma care are evolving to encompass attention to expeditious and safe management of not only multiply injured patients with these major fractures, but also definitive care for hip and periprosthetic fractures, which pose a similar burden of patient recumbency until stabilized. Future directions regarding refinement of patient resuscitation, assessment, and treatment are anticipated, as is the potential for data sharing and registries in enhancing trauma system functionality. Aims. Modular dual mobility (DM) prostheses in which a cobalt-chromium liner is inserted into a titanium acetabular shell (vs a monoblock acetabular component) have the advantage of allowing supplementary screw fixation, but the potential for corrosion between the liner and acetabulum has raised concerns. While DM prostheses have shown improved stability in patients deemed ‘high-risk’ for dislocation undergoing total hip arthroplasty (THA), their performance in young, active patients has not been reported. This study’s purpose was to assess clinical outcomes, metal ion levels, and periprosthetic femoral bone mineral density (BMD) in young, active patients receiving a modular DM acetabulum and recently introduced titanium, proximally coated, tapered femoral stem design. Patients and Methods. This was a prospective study of patients between 18 and 65 years of age, with a body mass index (BMI) < 35 kg/m. 2. and University of California at Los Angeles (UCLA) activity score > 6, who received a modular cobalt-chromium acetabular liner, highly crosslinked polyethylene mobile bearing, and cementless titanium femoral stem for their primary THA. Patients with a history of renal disease and metal hardware elsewhere in the body were excluded. A total of 43 patients (30 male, 13 female; mean age 52.6 years (. sd. 6.5)) were enrolled. All patients had a minimum of two years’ clinical follow-up. Patient-reported outcome measures, whole blood metal ion levels (ug/l), and periprosthetic femoral BMD were measured at baseline, as well as at one and two years postoperatively. Power analysis indicated 40 patients necessary to demonstrate a five-fold increase in cobalt levels from baseline (alpha = 0.05, beta = 0.80). A mixed model with repeated measures was used for statistical analysis. Results. Mean Harris Hip Scores improved from 54.1 (. sd. 20.5) to 91.2 (. sd. 10.8) at two years postoperatively (p < 0.001). All patients had radiologically well-fixed components, no patients experienced any instability, and no patients required any further intervention. Mean cobalt levels increased from 0.065 ug/l (. sd. 0.03) preoperatively to 0.30 ug/l (. sd. 0.51) at one year postoperatively (p = 0.01) but decreased at two years postoperatively to 0.16 ug/l (. sd. 0.23; p = 0.2). Four patients (9.3%) had a cobalt level outside the reference range (0.03 ug/l to 0.29 ug/l) at two years postoperatively, with values from 0.32 ug/l to 0.94 ug/l. The mean femoral BMD ratio was maintained in Gruen zones 2 to 7 at both one and two years postoperatively using this stem design. At two years postoperatively, mean BMD in the medial calcar was 101.5% of the baseline value. Conclusion. Use of a modular DM prosthesis and cementless, tapered femoral stem has shown encouraging results in young, active patients undergoing primary THA. Elevation in mean cobalt levels and the presence of four patients outside the reference range at two years postoperatively demonstrates the necessity of continued surveillance in this cohort. Aims. The purpose of this retrospective study was to evaluate the minimum five-year outcome of revision total hip arthroplasty (THA) using the Kerboull acetabular reinforcement device (KARD) in patients with Paprosky type III acetabular defects and destruction of the inferior margin of the acetabulum. Patients and Methods. We identified 36 patients (37 hips) who underwent revision THA under these circumstances using the KARD, fresh frozen allograft femoral heads, and reconstruction of the inferior margin of the acetabulum. The Merle d’Aubigné system was used for clinical assessment. Serial anteroposterior pelvic radiographs were used to assess migration of the acetabular component. Results. At a mean follow-up of 8.2 years (5 to 19.3), the mean Merle d’Aubigné score increased from 12.5 (5 to 18) preoperatively to 16.5 (10 to 18) (p < 0.0001). The survival rate at ten years was 95.3% (. sd. 4.5; 95% confidence interval (CI) 86.4 to 100) and 76.5% (. sd. 9.9, 95% CI 57.0 to 95.9) using aseptic loosening and radiological loosening as the endpoints, respectively. Conclusion. These results show that the use of the KARD with reconstruction of the inferior margin of the acetabulum in revision THA is associated with acceptable clinical results and survival at mid-term follow-up with, however, a high rate of migration of the acetabular component of 21.6%. This is a retrospective case review of 237 patients with displaced fractures of the acetabulum presenting over a ten-year period, with a minimum follow-up of two years, who were studied to test the hypothesis that the time to surgery was predictive of radiological and functional outcome and varied with the pattern of fracture. Patients were divided into two groups based on the fracture pattern: elementary or associated. The time to surgery was analysed as both a continuous and a categorical variable. The primary outcome measures were the quality of reduction and functional outcome. Logistic regression analysis was used to test our hypothesis, while controlling for potential confounding variables. For elementary fractures, an increase in the time to surgery of one day reduced the odds of an excellent/good functional result by 15% (p = 0.001) and of an anatomical reduction by 18% (p = 0.0001). For associated fractures, the odds of obtaining an excellent/good result were reduced by 19% (p = 0.0001) and an anatomical reduction by 18% (p = 0.0001) per day. When time was measured as a categorical variable, an anatomical reduction was more likely if surgery was performed within 15 days (elementary) and five days (associated). An excellent/good functional outcome was more likely when surgery was performed within 15 days (elementary) and ten days (associated). The time to surgery is a significant predictor of radiological and functional outcome for both elementary and associated displaced fractures of the acetabulum. The organisation of regional trauma services must be capable of satisfying these time-dependent requirements to achieve optimal patient outcomes

Purpose: Coxa magna is well known in Perthes’ disease but a quantitative evaluation of the early, in particular cartilaginous, enlargement of the femoral head and the necessary adaptive changes of the acetabulum (widening and/or growth) does not exist. We would like to present MR-based volumetric data. Methods: We measured the volume of the femoral head and the acetabulum in MRI by means of a software established for the sizing of tumours before therapy and the determination of liver lobe volume prior to transplantation. We evaluated MRI exams in 47 children with Perthes’ disease and 72 normal children from 4 to 9 years and present data of the affected hip in comparison to the unaffected hip and to normal hips. Results:. Femoral head:. On the average the affected head had a volume that was 47% (range 42 – 57%) larger than on the unaffected side and 44 % (range 13 – 59%) larger than in hips of healthy children. Cases with serial exams showed that the volume of the affected head increased in the course of time. Acetabulum:. On the average the acetabular volume was 21% (range 13 to 30%) larger on the affected side than on the unaffected side and 20% (range 10 to 29%) larger than in healthy children. In patients who underwent surgery (pelvic osteotomy, alone or together with intertrochanteric varus osteotomy) the acetabular volume was 24% larger (range 9 – 33%) on the affected side than on the unaffected side. In patients without surgery the acetabular volume was 16% larger (range 10 to 33%) on the affected side. Conclusions:. We found that Perthes’ disease is associated with an average increase of femoral head volume of 47% in comparison to the unaffected side and of 44% in comparison to healthy children. There was an average increase of the acetabular volume of 21% in comparison to the unaffected side and of 20% in comparison to healthy children. These data may allow a better understanding of the disease and a reappraisal of current forms of treatment. Significance: Given a chronic disproportion between the size of the femoral head and the acetabulum therapy should aim at:. Retardation of the (cartilaginous) enlargement of the femoral head. Promotion of widening or growth of the acetabulum. We believe that current conservative modes of treatment are effective through rationale A and B. Operative modalities, in particular pelvic osteotomies and/or intertrochanteric varus osteotomy, seem to be mainly effective through rationale B. By reorientation of the acetabulum and/or the proximal femur they should favour a better distribution of forces through the hip joint allowing for a gradual widening of the acetabulum. In addition, the operative trauma in the vicinity of the triradiate cartilage may have a stimulating effect on acetabular growth

Aims. The aim of this study is to evaluate whether acetabular retroversion (AR) represents a structural anatomical abnormality of the pelvis or is a functional phenomenon of pelvic positioning in the sagittal plane, and to what extent the changes that result from patient-specific functional position affect the extent of AR. Methods. A comparative radiological study of 19 patients (38 hips) with AR were compared with a control group of 30 asymptomatic patients (60 hips). CT scans were corrected for rotation in the axial and coronal planes, and the sagittal plane was then aligned to the anterior pelvic plane. External rotation of the hemipelvis was assessed using the superior iliac wing and inferior iliac wing angles as well as quadrilateral plate angles, and correlated with cranial and central acetabular version. Sagittal anatomical parameters were also measured and correlated to version measurements. In 12 AR patients (24 hips), the axial measurements were repeated after matching sagittal pelvic rotation with standing and supine anteroposterior radiographs. Results. Acetabular version was significantly lower and measurements of external rotation of the hemipelvis were significantly increased in the AR group compared to the control group. The AR group also had increased evidence of anterior projection of the iliac wing in the sagittal plane. The acetabular orientation angles were more retroverted in the supine compared to standing position, and the change in acetabular version correlated with the change in sagittal pelvic tilt. An anterior pelvic tilt of 1° correlated with 1.02° of increased cranial retroversion and 0.76° of increased central retroversion. Conclusion. This study has demonstrated that patients with symptomatic AR have both an externally rotated hemipelvis and increased anterior projection of the iliac wing compared to a control group of asymptomatic patients. Functional sagittal pelvic positioning was also found to affect AR in symptomatic patients: the acetabulum was more retroverted in the supine position compared to standing position. Changes in acetabular version correlate with the change in sagittal pelvic tilt. These findings should be taken into account by surgeons when planning acetabular correction for AR with periacetabular osteotomy. Cite this article: Bone Joint J 2024;106-B(2):128–135

Aims. Developmental dysplasia of the hip (DDH) describes a pathological relationship between the femoral head and acetabulum. Periacetabular osteotomy (PAO) may be used to treat this condition. The aim of this study was to evaluate the results of PAO in adolescents and adults with persistent DDH. Methods. Patients were divided into four groups: A, adolescents who had not undergone surgery for DDH in childhood (25 hips); B, adolescents who had undergone surgery for DDH in childhood (20 hips); C, adults with DDH who had not undergone previous surgery (80 hips); and D, a control group of patients with healthy hips (70 hips). The radiological evaluation of digital anteroposterior views of hips included the Wiberg angle (centre-edge angle (CEA)), femoral head cover (FHC), medialization, distalization, and the ilioischial angle. Clinical assessment involved the Harris Hip Score (HHS) and gluteal muscle performance assessment. Results. Significant improvements in radiological parameters were achieved in all measurements in all groups (p < 0.05). The greatest improvement was in CEA (mean of 19° (17.2° to 22.3°) in Group B), medialization (mean of 3 mm (0.9 to 5.2) in Group C), distalization (mean of 6 mm (3.5 to 8.2) in Group B), FHC (mean of 17% (12.7% to 21.2%) in Group B), and ilioischial angle (mean of 5° (2.3° to 8.1°) in Group B). There were significant improvements in the mean HHS and gluteal muscle performance scores postoperatively in all three groups. Conclusion. The greatest correction of radiological parameters and clinical outcomes was found in patients who had undergone hip surgery in childhood. Although the surgical treatment of DDH in childhood makes subsequent hip surgery more difficult due to scarring, adhesions, and altered anatomy, it requires less correction of the deformity and has a beneficial effect on the outcome of PAO in adolescence and early adulthood. We describe a little-known variety of hip dysplasia, termed ‘acetabular retroversion’, in which the alignment of the mouth of the acetabulum does not face the normal anterolateral direction, but inclines more posterolaterally. The condition may be part of a complex dysplasia or a single entity. Other than its retroversion, the acetabulum is sited normally on the side wall of the pelvis, and its articular surface is of normal extent and configuration. The retroverted orientation may give rise to problems of impingement between the femoral neck and anterior acetabular edge. We define the clinical and radiological parameters and discuss pathological changes which may occur in the untreated condition. A technique of management is proposed

Aims. The aims of this study were to validate the Forgotten Joint Score-12 (FJS-12) in the postoperative evaluation of periacetabular osteotomy (PAO), identify factors associated with joint awareness after PAO, and determine the FJS-12 threshold for patient-acceptable symptom state (PASS). Methods. Data from 686 patients (882 hips) with hip dysplasia who underwent transposition osteotomy of the acetabulum, a type of PAO, between 1998 and 2019 were reviewed. After screening the study included 442 patients (582 hips; response rate, 78%). Patients who completed a study questionnaire consisting of the visual analogue scale (VAS) for pain and satisfaction, FJS-12, and Hip disability and Osteoarthritis Outcome Score (HOOS) were included. The ceiling effects, internal consistency, convergent validity, and PASS thresholds of FJS-12 were investigated. Results. The median follow-up was 12 years (interquartile range 7 to 16). The ceiling effect of FJS-12 was 7.2%, the lowest of all the measures examined. FJS-12 correlated with all HOOS subscales (ρ = 0.72 to 0.77, p < 0.001) and pain and satisfaction-VAS (ρ = -0.63 and 0.56, p < 0.001), suggesting good convergent validity. Cronbach’s α was 0.95 for the FJS-12, which indicated excellent internal consistency. The median FJS-12 score for preoperative Tönnis grade 0 hips (60 points) was higher than that for grade 1 (51 points) or 2 (46 points). When PASS was defined as pain-VAS < 21 and satisfaction-VAS ≥ 77, the FJS-12 threshold that maximized the sensitivity and specificity for detecting PASS was 50 points (area under the curve (AUC) = 0.85). Conclusion. Our results suggest that FJS-12 is a valid and reliable assessment tool for patients undergoing PAO, and the threshold of 50 points may be useful to determine patient satisfaction following PAO in clinical settings. Further investigation of the factors influencing postoperative joint awareness may enable improved prediction of treatment efficacy and informed decision-making regarding the indication of PAO. Aims. The primary aim of this study is to assess the survival of the uncemented hydroxyapatite (HA) coated Trident II acetabular component as part of a hybrid total hip arthroplasty (THA) using a cemented Exeter stem. The secondary aims are to assess the complications, joint-specific function, health-related quality of life, and radiological signs of loosening of the acetabular component. Methods. A single-centre, prospective cohort study of 125 implants will be undertaken. Patients undergoing hybrid THA at the study centre will be recruited. Inclusion criteria are patients suitable for the use of the uncemented acetabular component, aged 18 to 75 years, willing and able to comply with the study protocol, and provide informed consent. Exclusion criteria includes patients not meeting study inclusion criteria, inadequate bone stock to support fixation of the prosthesis, a BMI > 40 kg/m. 2. , or THA performed for pain relief in those with severely restricted mobility. Results. Implant survival, complications, functional outcomes and radiological assessment up to ten years following index THA (one, two, five, seven, and ten years) will be performed. Functional assessment will include the Oxford Hip Score, Forgotten Joint Score, 12-Item Short Form Health Survey, EuroQol five-dimension health questionnaire, and pain and patient satisfaction. Radiological assessment with assess for acetabula lucent lines, lysis, and loosening according to DeLee and Charnley zones. Conclusion. This study is part of a stepwise introduction of a new device to orthopaedic practice, and careful monitoring of implants should be carried out as part of the Beyond Compliance principles. The results of this study will provide functional, radiological, and survival data to either support the ongoing use of the HA acetabulum or highlight potential limitations of this new implant before wide adoption. A secure bone/cement interface at the bone cement junction is an important requirement for the long-term success in the cemented hip arthroplasty. Cementing techniques have evolved and now involve pressurisation of the acetabulum and femur. It can be difficult to get a complete rim seal and hence adequate pressurisation due to the unique anatomy of the acetabulum and the contyloid notch. Several acetabular pressurisers are commercially available. We have developed an instrument for controlled and reproducible cement pressurisation in the acetabulum before socket insertion. It is a T-bar incorporating a central plunger, which protrudes from an outer sleeve when force is applied. The protrusion of the central plunger and hence the amount of force applied can be limited by a stop-sleeve. A laboratory saw bone model was designed to assess this system and compare it with two existing pressurisers. A polypropylene model of the acetabulum was used. Included in the model were two 1.3mm diameter capillary outlets, one at its pole and one at a point close to its rim opposite the cotyloid notch. Water was free to flow through the capillaries at a pressure of 13.5” WG to represent blood flow. 5 test per pressuriser were performed. CMW 1 Gentamicin bone cement was mixed as per manufacturers instruction in a Vacuum Mix system. The cement was then pressurised using one of three systems; the Depuy T handle pressuriser, the Exeter pressuriser and our new instrument. The cement mantle produced with the Depuy T-handle and the Exeter pressuriser was thicker at the pole than the rim and the cement intrusion was not consistent nor reproducible. The new pressurizer produced a cement mantle equal at the pole and the rim to within 1mm. A reproducible cement mantle compatible to the shape of the socket and with cement intrusion of 5mm (+/− 1mm) could be achieved. We recommend the use of this pressuriser

Various assumptions must be made during total hip replacement when placing the acetabular component within the reamed acetabulum with regards to its orientation. Various methods have been described for different acetabular components, some relying on knowledge of the existing anatomy and some relying on the use of jigs to align the component. Many of these assumptions are based on opinion rather than science and the current study was designed to define the position of the acetabulum in relation to a fixed position of the pelvis. A neutral position of the pelvis must be defined and this is represented by having a line from the anterior superior iliac spine to the pubis fixed in the coronal plane and taking measurements of the perpendicular axis of the pelvis taken from this. A variety of measurements were made using measurement techniques and the conclusion was reached that the average angle for operative inclination is 43 degrees ± 5 degrees and the average angle for operative anteversion is 29 degrees ± 8 degrees. These figures should give a preliminary guide to the correct placement of the acetabular socket in total hip replacement although other factors may need to be taken into account

We describe two patients aged 16 and 25 years with osteogenesis imperfecta who sustained displaced fractures of the acetabulum following minor trauma. The femoral heads were deformed by impact against the acetabular margin and both cases underwent surgical reconstruction. The quality of the bone and soft tissues made the operations challenging. There were potential complications specific to osteogenesis imperfecta, including bleeding, the creation of secondary fracture lines and shredding of the soft-tissue. The cases provide useful guidelines for addressing these difficulties

Aims. Slipped upper femoral epiphysis (SUFE) has well documented biochemical and mechanical risk factors. Femoral and acetabular morphologies seem to be equally important. Acetabular retroversion has a low prevalence in asymptomatic adults. Hips with dysplasia, osteoarthritis, and Perthes’ disease, however, have higher rates, ranging from 18% to 48%. The aim of our study was to assess the prevalence of acetabular retroversion in patients presenting with SUFE using both validated radiological signs and tomographical measurements. Methods. A retrospective review of all SUFE surgical cases presenting to the Royal Children’s Hospital, Melbourne, Australia, from 2012 to 2019 were evaluated. Preoperative plain radiographs were assessed for slip angle, validated radiological signs of retroversion, and standardized postoperative CT scans were used to assess cranial and mid-acetabular version. Results. In all, 116 SUFEs presented in 107 patients who underwent surgical intervention; 47 (52%) were male, with a mean age of 12.7 years (7.5 to 16.6). Complete radiological data was available for 91 patients (99 hips) with adequate axial CT imaging of both hips. Overall, 82 patients (82%) underwent pinning in situ (PIS), with subcapital realignment surgery (SRS) performed in 17 patients (18%) (slip angles > 75°). Contralateral prophylactic PIS was performed in 72 patients (87%). On the slip side, 62 patients (68%) had one or more radiological sign of retroversion. Tomographical acetabular retroversion was more pronounced cranially than caudally of the acetabulum on both the affected side and the contralateral side (p < 0.001) as expected in the normal population. Increasing severity of the slip was found to be directly proportional to the degree of reduction in cranial and central acetabular version (p < 0.05) in the SUFE hips. Conclusion. Acetabular retroversion is more prevalent in patients with SUFE than previously reported, and have been shown be correlated to the severity of the slip presentation. The presence of radiological signs of acetabular retroversion could be used to justify prophylactic contralateral pinning. Cite this article: Bone Jt Open 2022;3(2):158–164

Aims. Periacetabular osteotomy (PAO) is widely recognized as a demanding surgical procedure for acetabular reorientation. Reports about the learning curve have primarily focused on complication rates during the initial learning phase. Therefore, our aim was to assess the PAO learning curve from an analytical perspective by determining the number of PAOs required for the duration of surgery to plateau and the accuracy to improve. Methods. The study included 118 consecutive PAOs in 106 patients. Of these, 28 were male (23.7%) and 90 were female (76.3%). The primary endpoint was surgical time. Secondary outcome measures included radiological parameters. Cumulative summation analysis was used to determine changes in surgical duration. A multivariate linear regression model was used to identify independent factors influencing surgical time. Results. The learning curve in this series was 26 PAOs in a period of six months. After 26 PAO procedures, a significant drop in surgical time was observed and a plateau was also achieved. The mean duration of surgery during the learning curve was 103.8 minutes (SD 33.2), and 69.7 minutes (SD 18.6) thereafter (p < 0.001). Radiological correction of acetabular retroversion showed a significant improvement after having performed a total of 93 PAOs, including anteverting PAOs on 35 hips with a retroverted acetabular morphology (p = 0.005). Several factors were identified as independent variables influencing duration of surgery, including patient weight (β = 0.5 (95% confidence interval (CI) 0.2 to 0.7); p < 0.001), learning curve procedure phase of 26 procedures (β = 34.0 (95% CI 24.3 to 43.8); p < 0.001), and the degree of lateral correction expressed as the change in the lateral centre-edge angle (β = 0.7 (95% CI 0.001 to 1.3); p = 0.048). Conclusion. The learning curve for PAO surgery requires extensive surgical training at a high-volume centre, with a minimum of 50 PAOs per surgeon per year. This study defined a cut-off value of 26 PAO procedures, after which a significant drop in surgical duration occurred. Furthermore, it was observed that a retroverted morphology of the acetabulum required a greater number of procedures to acquire proficiency in consistently eliminating the crossover sign. These findings are relevant for fellows and fellowship programme directors in establishing the extent of training required to impart competence in PAO. Purpose: The distribution of weight bearing area within the acetabulum is of importance in addressing trauma to the acetabulum, hip joint deformities and causes of osteoarthritis. According to Wolf’s law, bone density can indicate loading patterns experienced. The objective of this study was to characterize distributions of acetabular bone density patterns by regions in the normal population. Method: CT scans of 22 subjects, mean age 70.6 with no evidence to hip joint pathologies were analysed. Bone density distribution maps were generated within AmiraDEV4.1 image analysis software using custom written plugins (Visage Imaging, Carlsbad, USA). Acetabular cup surfaces were semi-automatically segmented from the reconstructed CT volumes with an atlas-based approach. The acetabular cups were expanded 2.5 mm into the acetabular bone, and surface bone densities were calculated as the average bone density within ±2.5mm. The distribution maps were analysed using zones to spatially classify areas of high and low bone density in a healthy population. The acetabular cups were aligned using the acetabular rim plane that was landmarked, and by rotating the cups, such that a 900 abduction angle and a 00 anteversion angle were achieved. The grid used was divided to quadrants, and subdivided into radial thirds of the average rim radius. The correspondence of left and right density maps was investigated by comparing the average bone density in corresponding zones and across the population. Results: High bone densities were found around the roof of the acetabulum aligning with the femoral mechanical axis during standing. The highest average bone density were found to be the superior and posterior walls of the acetabulum, corresponding to regions 8, 9, and 12 compared to other regions of the acetabuli (P< 0.01). A strong correlation was found between left and right sides within subjects (R=0.91, P< 0.05); and weaker correlation was also found for overall average bone density, (R=0.77, P< 0.05). Conclusion: The location of the zones with the highest average bone density agrees with cadaveral studies of the maximum contact stress in the acetabulum (zones 9 and 12). [1,2]. It may explain why trauma to these areas carries a higher risk for early arthritic changes

The JRI cemented total hip replacement consisted of an acetabulum with a gamma irradiated UHMW polyethylene cup encased in a titanium metal shell, a 32 mm titanium head, and forged titanium alloy stem. Revision of this implant for failure, in particular that of the acetabulum, and the lack of published survivorship prompted a retrospective review of the local series. Data was compiled from theatre log books, patient records and X-rays. During the period 1989–1997 a single surgeon performed 304 total hip replacements using this JRI prosthesis through an anterior lateral approach. The average age was 70 (48–96), with 186 (61%) female and 85% performed for osteoarthritis. Complete data was available in 236 cases to allow further analysis. The average follow up was 9.3 (1–17) years. During this period 37 hips had to be revised: 28 for a loose acetabulum, both components were loose in 5, and 3 had revision for infection. The mean time to revision was 7.4 (1–17) years. This data provides a revision rate of at least 16% at ten years. This data does not take potential earlier radiographic failure nor patient function into account. This prosthesis therefore fails to comply with modern benchmarks for survivorship of implants. Furthermore this study highlights the importance of ongoing review of patients who have undergone joint replacement, in particular those with implants that have problems with design, as has been demonstrated in other metal backed components

Our study was designed to compare the effect of indometacin with that of a placebo in reducing the incidence of heterotopic ossification in a prospective, randomised trial. A total of 121 patients with displaced fractures of the acetabulum treated by operation through a Kocher-Langenbeck approach was randomised to receive either indometacin (75 mg) sustained release, or a placebo once daily for six weeks. The extent of heterotopic ossification was evaluated on plain radiographs three months after operation. Significant ossification of Brooker grade III to IV occurred in nine of 59 patients (15.2%) in the indometacin group and 12 of 62 (19.4%) receiving the placebo. We were unable to demonstrate a statistically significant reduction in the incidence of severe heterotopic ossification with the use of indometacin when compared with a placebo (p = 0.722). Based on these results we cannot recommend the routine use of indometacin for prophylaxis against heterotopic ossification after isolated fractures of the acetabulum

Fracture of the acetabulum can lead to degenerative arthritis of the hip, avascular necrosis of the femoral head, or both. Total hip arthroplasty is a common form of surgical treatment when significant joint changes and pain are present. Ten patients with fracture acetabulum were treated in this study using metal on metal total hip arthoplasty. The initial fracture was posterior wall fracture in one patient, posterior column fracture in one patient, transverse fracture in 2 patients, fracture dislocation in 3 patients and fracture posterior wall and column in 3 patients. The indications of arthroplasty were secondry osteoarthritis after internal fixation or after conservative management or collapse of the femoral head. Arthroplasty was done after an average period of 1.8 years (range from 1 to 4 years). After a follow up period ranged from 3 to 7 years with a mean of 4.6 years, the Harris hip score was improved from a mean of 51 (range 20 to 65) to a mean of 92.5 (range 90 to 95). Infection occurred in one case and two stages revision was done. Another case developed loosening of the acetabular component and was revised using cementless cup fixed with screws and bone graft. Metal on metal THR after acetabular fracture are relatively uncomplicated and lead to a good outcome despite the difficulties faced during the procedure

Aims. In the native hip, the hip capsular ligaments tighten at the limits of range of hip motion and may provide a passive stabilizing force to protect the hip against edge loading. In this study we quantified the stabilizing force vectors generated by capsular ligaments at extreme range of motion (ROM), and examined their ability to prevent edge loading. Methods. Torque-rotation curves were obtained from nine cadaveric hips to define the rotational restraint contributions of the capsular ligaments in 36 positions. A ligament model was developed to determine the line-of-action and effective moment arms of the medial/lateral iliofemoral, ischiofemoral, and pubofemoral ligaments in all positions. The functioning ligament forces and stiffness were determined at 5 Nm rotational restraint. In each position, the contribution of engaged capsular ligaments to the joint reaction force was used to evaluate the net force vector generated by the capsule. Results. The medial and lateral arms of the iliofemoral ligament generated the highest inbound force vector in positions combining extension and adduction providing anterior stability. The ischiofemoral ligament generated the highest inbound force in flexion with adduction and internal rotation (FADIR), reducing the risk of posterior dislocation. In this position the hip joint reaction force moved 0.8° inbound per Nm of internal capsular restraint, preventing edge loading. Conclusion. The capsular ligaments contribute to keep the joint force vector inbound from the edge of the acetabulum at extreme ROM. Preservation and appropriate tensioning of these structures following any type of hip surgery may be crucial to minimizing complications related to joint instability. Aims. Perthes’ disease is an idiopathic avascular necrosis of the developing femoral head, often causing deformity that impairs physical function. Current treatments aim to optimize the joint reaction force across the hip by enhancing congruency between the acetabulum and femoral head. Despite a century of research, there is no consensus regarding the optimal treatment. The aim of this study was to describe the experiences of children, their families, and clinicians when considering the treatment of Perthes’ disease. Methods. A qualitative study gathered information from children and their families affected by Perthes’ disease, along with treating clinicians. Interviews followed a coding framework, with the interview schedule informed by behavioural theory and patient and public involvement. Transcripts were analyzed using the framework method. Results. A total of 24 interviews took place, with 12 child/family dyads and 12 clinicians from UK NHS centres. Interviews identified widespread variation of routine care. Children/their families recounted positive experiences when included in the decision-making process for treatment. There is a strong desire from clinicians and children/families for consistent guidance from everyone involved in care, which should be based on clinical consensus. Conclusion. This is the first study to describe how children/families and clinicians experienced receiving or providing treatment in Perthes’ disease. The results indicate the need for robust evidence to support treatment decisions. Children and families valued feeling involved in the clinical decision-making process. Clinicians acknowledged the central importance of providing patient-centred care, particularly in the absence of robust evidence to guide the optimal treatment decisions. This study will inform a future Delphi project to develop clinical consensus guidelines for the treatment of Perthes’ disease. Cite this article: Bone Jt Open 2023;4(10):735–741

Aims. The aim of this study was to understand the experience of mature patients who undergo a periacetabular osteotomy (PAO), a major hip-preserving surgical procedure that treats symptomatic hip dysplasia by realigning the acetabulum. Our aim was to improve our understanding of how the operation affected the lives of patients and their families, with a long-term goal of improving their experience. Methods. We used a phenomenological approach with in-depth, semi-structured interviews to investigate the experience of seven female patients, aged between 25 and 40 years, who underwent a PAO. A modified homogeneity sampling approach coupled with criterion sampling was used. Inclusion criteria involved having at least one child at home and being in a committed relationship with a spouse or partner. Results. Analysis of interview transcripts revealed five major themes: feelings of frustration before having a treatment plan; fear of the operation; the importance of understanding the procedure in order to feel in control of their care; feelings of helplessness, dependence, and depression during recovery; and the need for support during recovery. Conclusion. Our findings suggest that both the physical and psychosocial burdens of undergoing a PAO are great, with a major impact on the patient and their family. Over the past 40 years, the management of displaced fractures of the acetabulum has changed from conservative to operative. We have undertaken a meta-analysis to evaluate the classification, the incidence of complications and the functional outcome of patients who had undergone operative treatment of such injuries. We analysed a total of 3670 fractures. The most common long-term complication was osteoarthritis which occurred in approximately 20% of the patients. Other late complications, including heterotopic ossification and avascular necrosis of the femoral head, were present in less than 10%. However, only 8% of patients who were treated surgically needed a further operation, usually a hip arthroplasty, and between 75% and 80% of patients gained an excellent or good result at a mean of five years after injury. Factors influencing the functional outcome included the type of fracture and/or dislocation, damage to the femoral head, associated injuries and co-morbidity which can be considered to be non-controllable, and the timing of the operation, the surgical approach, the quality of reduction and local complications which are all controllable. The treatment of these injuries is challenging. Tertiary referrals need to be undertaken as early as possible, since the timing of surgery is of the utmost importance. It is important, at operation, to obtain the most accurate reduction of the fracture which is possible, with a minimal surgical approach, as both are related to improved outcome

We have divided Severin group-V severely dysplastic hips with a false into three subtypes, based on the height and shape of the socket. We performed rotational acetabular osteotomy (RAO) in 19 hips in 17 young adults with a type-1 ‘low’ false acetabulum which had direct contact with the true acetabulum. This is a periacetabular osteotomy which gives acetabular coverage with articular cartilage and produces a nearly normal position of the head. Concomitant osteotomies of the proximal femur were carried out in 11 hips. We reviewed the patients clinically and radiologically at a mean of ten years (6 to 18) after operation. Of the 19 hips, 15 showed very good or good results. This operation is indicated in young adults with a dysplastic hip and a type-1 low false acetabulum. Subclassification of Severin group V is a convenient way of defining those patients who would benefit from the procedure

Our aim was to evaluate the efficacy of a two-level reconstruction technique using subchondral miniscrews for the stabilisation of comminuted posterior-wall marginal acetabular fragments before applying lag screws and a buttress plate to the main overlying posterior fragment. Between 1995 and 2003, 29 consecutive patients with acute comminuted displaced posterior-wall fractures of the acetabulum were treated operatively using this technique. The quality of reduction measured from three standard plain radiographs was graded as anatomical in all 29 hips. The clinical outcome at a mean follow-up of 35 months (24 to 90) was considered to be excellent in five patients (17%), very good in 16 (55%), good in six (21%) and poor in two (7%). The use of the two-level reconstruction technique appears to provide stable fixation and is associated with favourable results in terms of the incidence of post-traumatic osteoarthritis and the clinical outcome. However, poor results may occur in patients over the age of 55 years

The effects of NF-I on the hip have been underreported in literature. The bony changes in the hip can be mild to severe and are often present, but not diagnosed. Dislocation of the hip has been described but protrusio acetabulum is underdiagnosed and has only been reported from one institute. This is the first case where an open biopsy has been taken from the hip joint by surgical dislocation with preservation of the blood supply to the femoral head. Gross and microscopic pathology in the neurofibromatosis of the hip has been described for the first time in our report. A 16-year-old girl presented with history of pain, limp and stiffness in the right hip for several years. Radiographs, CT scan showed severe protrusio acetabulum. Histological analysis from the tissue taken from the hip was a neurofibroma. There was very minimal hyaline cartilage in the acetabulum at operation. Therefore the floor of the acetabulum was grafted with the autologous cancellous bone chips obtained from her greater trochanter. The aim of this was to lateralize the hip center to a normal position. At six months follow up she was doing well. Her range of movements had increased by 20%

Previous work has demonstrated vulnerability of the femoral nerve to damage by anterior acetabular retractors during THA. The aim of this study was to quantify the proximity of the femoral nerve to the anterior acetabulum, on cadaveric material and MRI studies. A standard posterior approach to the hip was carried out in 6 fresh frozen cadaveric hemipelves. Following dislocation and removal of the femoral head, measurements were taken from the anterior acetabular lip to the posterior aspect of the femoral nerve as it passed over this point. 14 MRI studies of the hip were obtained from the local PACS database (7 male, 7 female; mean age 58 (range 32–80)). T1 weighted axial scans were reviewed. Measurements were obtained from the anterior acetabular lip to the posterior surface of the femoral nerve and artery, and the cross-sectional area of iliopsoas was calculated. There was no significant difference between the mean distances to the femoral nerve in the cadaveric (24 mm) and MRI groups (25.3mm) (p=0.7). On MRI images, the distance between the acetabular wall and both the femoral artery (p=0.003) and femoral nerve (p=0.007) was significantly larger in men. The femoral artery is strikingly close to the acetabulum in females, passing a mean distance of 14.8 mm, whereas in males this was 23.9 mm. The mean femoral nerve distance was 28.7 mm in males and 21.9 mm in females. The cross-sectional area of iliopsoas was significantly smaller in women (5.97 cm. 2. compared to 11.37 cm. 2. , p<0.001). Both the femoral artery and nerve run in close proximity to the anterior acetabular lip. Care should be taken when placing instruments in this area to avoid neurovascular injury. The increased incidence of femoral nerve damage in women following THA may be due to the significantly smaller bulk of iliopsoas

Purpose: The surgical approach is an essential element for precise reduction and rigid fixation of fractures of the acetabulum. In cases where the anterior column is mainly involved and total hip arthroplasty (THA) is indicated, classical approaches do not allow double assess to the anterior column necessary for reduction and fixation and to the femoral canal for insertion of the prosthesis. Combining Heuter’s anterior approach and Letournel’s ilio-femoral approach provides access to the anterior column and to the often associated posterior hemi-transverse fracture, while allowing insertion of the total hip prosthesis. Material and method: This combined approach was used for ten among 60 THA performed for fracture of the acetabulum. Seven were primary procedures and three were differed reconstruction procedures. Mean patient age was 60.6 years (range 50 – 85 years). Fracture types were: acetabular wall and anterior column (n=8), anterior column and posterior hemi-transverse (n=2). All fresh fractures exhibited major acetabular damage associated with fracture of the femoral neck. A hybrid THA was used for all cases after fixation of the fracture. The femoral heads were used as graft material for deficient anterior columns in two patients and as “piecemeal” grafts for the others. Results: Mean follow-up was 36 months (range 24–35 months). At last follow-up all fractures had healed and all acetabular components exhibited solid fixation with no sign of migration. The mean outer diameter of the ace-tabular components inserted was 56 mm (52–64) using a 2 mm press-fit and one screw. The only complication was one postoperative anterior dislocation. Pain relief and function were satisfactory in all patients at last follow-up with a Postel Merle d’Aubigné score of 16 (13–18). Discussion: This anterior surgical approach enables good access to the acetabular walls and anterior columns, allowing solid fixation and relatively easy THA. The Kocher-Langenbech approach is still better in case of posterior deficiency or when posterior fixation requires space for inserting an acetabular component. We reserve it for THA used to treat fresh fractures of the acetabulum with major damage to the acetabulum and/or the femoral head, with or without neck fracture in patients aged 55 years or more. For differed reconstruction, we also use this approach if the acetabular damage involves the anterior column

Purpose: The use of femural head as an autograft in the reconstruction of dysplastic acetabulum is a fast, convinient and with excellent results throught time. Materlal-method: Since January 1986 until December 2002, 57 total hip replacements had taken place for dysplastic acetabulum. In all of them, the femural head has been used as an autograft. The age of the patients was between 42 to 75 years old (m.a. 63 ñ.Ï.). 45 of them were females and 12 males. 9 patients have been suggested to intertrïchanderic osteotomy in the past. One third of the patients had also the other hip affected. The surgical approach to the hip in all cases was made by the Harding’ s techique. The autograft in received from the femural neck. We prepared the acetabulum until we had bleeding bone. The bone graft is beared with two screws and if any gup exist is filled with cancellous autografts. In 41 operations of total hip replacements the cup was fixed with cement and in the rest 16 without. In our research we have included 43 hips with a follow-up from 3 to 14 years (m.a. 8,5 y.o.). Results: The results were evaluated with the Merle d’Aubigne and Postel scale. 5.6 pain, 4.1 wirdth of mobility. The callousness and the remodeling of the autograft were coplet in x-rays control. Íï case of absorbe of the bone graft. Until now no artrhoplasty has been revised. Conclusions: We consider that this is a satisflng method for filling and supporting the cup and in adittion it offers advantages in case that the artrhoplasty has to be revised

The juvenile age group was investigated clinically and biochemically but no causative metabolic factor was found. 3. Transposition osteotomy of the acetabulum (TOA) was the first periacetabular osteotomy for the osteoarthritis hips due to acetabular dysplasia, in which the acetabulum was transposed with articular cartilage. TOA improves coverage of the femoral head to restore congruity and stability, and also prevent further osteoarthritis deterioration and induce regeneration of the joint. Many good clinical outcomes have been reported for such periacetabular osteotomies for osteoarthritis of the hips at an early stage. In contrast, the clinical outcome is controversial for those hips at an advanced stage, in which the joint space has partly disappeared. The purpose of this study was to investigate whether TOA is an appropriate option for treatment of osteoarthritis of the hips at the advanced stage by comparing with matched control hips at the early stage. Between 1998 and 2001, TOA was performed in 104 hips of 98 patients. Sixteen of 17 hips (94%) with osteoarthritis at the advanced stage were examined and compared with 37 matched control hips at the early stage. The mean age at the operation was 48(38–56) and the mean follow-up period was 88 (65–107) months. TOA corrected the acetabular dysplasia and significantly improved containment of the femoral head. No hips had secondary operations including THA. Clinical scores were also significantly improved in both of the groups. In the advanced osteoarthritis cases, there was a tendency for abduction congruity before transposition osteotomy of the acetabulum to reflect the clinical outcome. TOA is a promising treatment option for the advanced osteoarthritis of the hips as well as for those patients at the early stage when preoperative radiographs show good congruity or containment of the joint

Introduction: Revision hip surgery is predicted to rise significantly over the coming decades. There is therefore likely to be an increasing need to overcome the large bone loss and cavitatory defects encountered in failed primary hip replacements. Impaction bone grafting (IBG) is a recognised technique for replacing lost bone stock. Achieving optimal graft impaction is a difficult surgical skill with a significant learning curve, balancing the need to achieve sufficient compaction to provide primary stability versus the need to keep impaction forces to a minimum to prevent iatrogenic fracture. In this study we have developed a revision acetabular model to test the hypothesis that the use of vibration and drainage with a new custom made perforated tamp could reduce the peak stresses imparted to the acetabulum during the impaction process and also improve the reliability and reproducibility of the impaction technique. Methods: Composite Sawbone hemi Pelvis models were used, with identical contained cavitatory defects created (Paprosky Type 2a). A strain gauge was attached to the medial wall of each hemi pelvis. A custom set of IBG tamps were made, and coupled a pneumatic hammer used to generate the vibrations. A standard impaction technique was used for the control group and the new vibration impaction for the experimental group. The cavity was progressively filled with morsellised allograft in 6 set steps for both groups with strain gauge readings taken during all impaction to monitor peak stresses. A standard Exeter Contemporary cup was then cemented into the graft bed for both groups. The models were mechanically loaded according to the protocol developed by Westphal et al at the angle of the joint reaction force during heel strike for a total of 50 000 cycles. 3D assessment of any micro motion post mechanical testing and degree of graft compaction was done with high resolution micro CT. Results: Vibration impaction lead to a significant reduction in the peak stresses during the impaction process throughout the 6 steps (e.g. Step 1: 34.6 vs 110.8 MPa p=0.03). There was also far less variability in the peak stresses in the vibration group compared to standard impaction both in sequential impactions by the same surgeon and between different surgeons. One medial wall fracture occurred in the control group only. There was no difference in the degree of graft compaction or in the subsidence of the implant post cyclical loading. Conclusion: Impaction bone grafting can be a difficult surgical skill with a significant learning curve. We believe that this new technique of applying vibration coupled with drainage to the IBG process in the acetabulum can reduce the risk of intraoperative fracture whilst achieving good graft compaction and implant stability. This technique therefore has the potential to widen the ‘safety margins’ of IBG and reduce the learning curve allowing more widespread adoption of the technique for replacing lost bone stock

The outcome of complex acetabular reconstruction was evaluated in twenty-one patients who were confined to a wheelchair or bed because of pain from acetabular metastases. Reconstruction rings were used where bone loss exceeded 50% of the acetabulum. Six roof reinforcement-rings, eight ilioischial-rings and eight Harrington reconstructions were performed. All but two patients(90%) became ambulatory without pain. Median survival was nine months. Two patients underwent acetabular revision for recurrence. These results support the role of acetabular reconstruction for palliation of pain in appropriate patients with acetabular metastases. Metastatic disease of the acetabulum is painful and disabling. Operative intervention is indicated in certain patients with pathologic fractures, and non-responders to adjuvant treatment. The functional outcome of hip arthroplasty with reconstruction rings was evaluated in twenty-one patients with acetabular metastases between 1989 and 2001. Preoperatively all patients were confined to a wheelchair or bed and used significant narcotic medications. Preoperative radiotherapy was employed in eighteen cases (90%) and 30% had undergone chemotherapy. AAOS classification of the acetabular lesion revealed: six-type II, seven-type III and eight-type IV deficiencies. All cases required a reconstruction ring due to bone loss exceeding 50% of the acetabular dome. Six roof reinforcement rings, eight ilioischial rings and eight Harrington reconstructions with rings were performed in this group. Determination of the reconstructive technique was based on preoperative computerized tomography and intraoperative examination of the acetabular deficiency. All but two patients (90%) became ambulatory without significant pain. Eleven patients used a walker or two canes and nine walked with one or no canes. Median survival was nine months and patients with visceral involvement had a shorter duration. Eight early post-operative complications developed in six patients (29%). In two patients the acetabular construct failed with cup migration due to locally recurrent disease; both were successfully revised. The results of complex acetabular reconstruction for metastatic disease validate its role for palliation of pain and to improve ambulatory status. Preoperative planning with computerized tomography can assist in classifying acetabular bone loss and determining optimal reconstruction technique

To assess clinical outcomes, metal ion levels, and periprosthetic femoral bone mineral density (BMD) in young, active patients receiving a modular dual mobility acetabulum and recently introduced titanium, proximally coated, tapered femoral stem design. This was a prospective study of patients  65 years of age, with a BMI  35 kg/m. 2. , and UCLA activity score > 6 who received a modular cobalt chrome acetabular liner, highly cross-linked polyethylene mobile bearing, and cementless titanium femoral stem for their primary THA. Patients with a history of renal disease and metal hardware elsewhere in the body were excluded. All patients had a minimum of 2-year clinical follow-up. Patient reported outcome measures, whole blood metal ion levels (ug/L), and periprosthetic femur BMD were measured at baseline and at 1- and 2-years postoperatively. 43 patients (30 male, 13 female; mean age 52.6 ± 6.5 years) were enrolled. Harris Hip Scores improved from 54.1 ± 20.5 to 91.2 ± 10.8 at 2 years postoperatively (p<0.001). All patients had radiographically well-fixed components, no patients have sustained an instability event, and no patients have required a return to the operating room or revision procedure. Mean cobalt levels increased from 0.065 ± 0.03 ug/L preoperatively to 0.30 ± 0.51 at 1-year postoperatively (p=0.01), but decreased at 2 years postoperatively to 0.16 ± 0.23 (p=0.2) (Table 1). Four patients (9.3%) had a cobalt level outside the reference range (0.03 to 0.29ug/L) at 2 years postoperatively with values from 0.32 to 0.94. None were symptomatic. The mean femoral BMD ratio was maintained in Gruen zones 2 thru 7 at both 1- and 2-years postoperatively using this stem design (Table 2). At 2 years postoperatively, BMD in the medial calcar was 101.5% of the baseline value. Use of a modular dual mobility prosthesis and cementless, tapered femoral stem has shown encouraging results in young, active patients undergoing primary THA. Elevation in mean cobalt levels and the presence of four patients outside the reference range at 2 years postoperatively demonstrates the necessity of continued surveillance in this cohort. Aim: The purpose of this work is to report the early results from medial protrusio technique for placement of acetabular component without cement in patients who have acetabular dysplasia. Material and Method: Thirty-two hip replacements were performed in thirty consecutive cases with dysplastic osteoarthritis between 2001 and 2003. In none of the patients an augmentation with bone or cement of the superolateral aspect of the acetabulum was made. Indication for operation was a painful hip joint that could not be alleviated by conservative treatment. According to Crowe’s criteria the dysplasia was evaluated as type I (19); type II (11); type III (3). The size and localization of the true acetabulum were evaluated using Ranawat’s triangle on a weigh-bearing preoperative X-ray. Clinical appraisal of the joint was done preoperatively, at the sixth, twelfth and thirty-six month postoperatively by the scheme of Merle dAubigne and Postel modified by Charnley. In 29 cases the surgery was performed via lateral transgluteal approach of Hardinge-Mulliken and in the rest of the cases through anterolateral approach of Watson-Jones. In all cases the medial wall was perforated with reaming and the medial periosteum was torn to visualize the illiacus muscle. Results: One to three-year postoperatively clinical results showed significant improvement: in 28 joints 15–18 points (excellent and very good results); in 5 joints 14–16 points (good result). The medialization of the acetabular component is found to be 5.72.8 mm average values. X-ray evidences for medial migration of the acetabular component and early loosening were not found. Nerve palsy and intraoperative fracture of the femur did not occur, but one joint dislocated 24 days after the replacement despite the resection of anterior inferior iliac spine. Excessive medialization and impingement of the femur to the pelvic bone were the reasons for this complication. Conclusion: When precisely planned the medial protrusion technique without cement fixation of the acetabu-lar component is a good alternative for arthroplasty in dysplastic hip joint. Particular attention must be paid in preserving enough thickness of the anterior and posterior acetabular walls during the reaming process. The perforation of the medial wall must not exceed 25–30% from the surface, because of plain risk of protrusion of the component beyond the teardrop figure of Kohler

Pathological fractures due to metastasis with destruction of the acetabulum and central dislocation of the hip present a difficult surgical challenge. We describe a series using a single technique in which a stable and long-lasting reconstruction was obtained using standard primary hip replacement implants augmented by strong, fully-threaded steel rods with cement and steel mesh, where required. Between 1997 and 2006, 19 patients with a mean age of 66 years (48 to 83) were treated using a modified Harrington technique. Acetabular destruction was graded as Harrington class II in six cases and class III in 13. Reconstruction was achieved using three 6.5 mm rods inserted through a separate incision in the iliac crest followed by augmentation with cement and a conventional cemented Charnley or Exeter primary hip replacement. There were no peri-operative deaths. At the final follow-up (mean 25 months (5 to 110)) one rod had fractured and one construct required revision. Of the 18 patients who did not require revision, 13 had died. The mean time to death was 16 months (5 to 55). The mean follow-up of the five survivors was 31 months (18 to 47). There were no cases of dislocation, deep infection or injury to a nerve, the blood vessels or the bladder

We carried out a morphometric analysis of the acetabulum following Dega osteotomy in patients with cerebral palsy using three-dimensional CT. We assessed 17 acetabula in 12 patients with instability of the hip. A Dega osteotomy and varus derotation femoral osteotomy were performed in all 17 hips. Three-dimensional CT scans were taken before and approximately one year after operation. Acetabular cover was evaluated using anterosuperior, superolateral and posterosuperior acetabular indices, and the change in the acetabular volume was calculated. Inter- and intra-observer reliability was assessed using the intraclass correlation coefficient. After the osteotomy, the anterosuperior, superolateral and posterosuperior cover had improved significantly towards the value seen in a control group. The mean acetabular volume increased by 68%

Acetabular components of total hip joint replacement (THJR) in previously irradiated pelvis show high rates of failure. We present a literature review and a retrospective series evaluating the survival of acetabular cages in this difficult situation. Our hypothesis was that cage reinforcement of the acetabulum after previous pelvic irradiation would lead to early failure. A cohort of 11 patients (12 hips) was identified, who had undergone THJR utilising an acetabular cage, after previous pelvic irradiation for malignant tumours. All operations were performed by a single surgeon in Waikato over the period of 1997–2007. Six patients (six hips) died within one year of their operation, the further five patients (six hips) were analysed for survival and radiograpical loosening of the acetabular component. Complications attributed to previous irradiation are also reported. There is a paucity of literature of THJR survivorship after pelvic irradiation. The first series from the 1970’s showed 50% acetabular loosening at 5 years in cemented cups. Two conflicting series are published with 44% vs. 0% failure of uncemented cups. Only one previous series (22 hips) reports the use of acetabular reinforcement rings, and showed a 20% loosening and 10% deep infection rate at 4 years. In our cohort of 12 hips in 11 patients, only five patients survived greater than one year after joint replacement. The average follow up of the remaining six hips is five years (two to ten years). Two out of six of the acetabular cages have catastrophically failed. Of the remaining four cages, one is probably, and three are possibly radiologically loose. Two out of six have raised concerns in regards to deep infection that were not proven microbiologically. Overall of the patients who survived greater than one year after their THJR for pelvic irradiation, only four out of six of the acetabular cages remained insitu, and all had concerns raised in regards to radiographical loosening. We report a high rate of clinical and radiographic failure of cage reconstruction of the acetabulum after previous pelvic irradiation. A superior method of acetabular reconstruction in this difficult situation is yet to emerge. Alternative methods of reconstruction continue to develop, with trabecular metal options a possible consideration

Background: It is thought that the forces transmitted across the hip joint produce migration of the prosthesis by failure at either the bone-cement or the prosthesis-cement interface. As symptoms associated with such motions often result from failure at the cement-bone interface, it is this interface and its sub-surfaces that are the critical areas of prosthesis loosening. Our aim is to produce a new and more accurate method of measuring strains at this critical interface. Objective: To develop in-vitro experiments to measure the strain distributions near the bone-cement interface of the acetabulum region under physiological, quasi-static loading conditions. Experimental Model: Two hemi-pelvic specimens of saw bones were used. Following careful placement of six protected precision strain gauges (4.6 x 6.4mm, tri-axial EA-13-031RB-120/E). One specimen was prepared to receive a cemented polyethylene cup (Depuy Charnley Ogee LPW 53/22). An uncemented 58mm Duraloc cup was implanted into a second specimen. Methods: Hip joint force relative to the cup during normal walking (Bergmann, G., 2001. HIP98) was used for quasi-static tests on a Llody LR30K loading machine. The magnitude of the maximum and minimum principal strains, and the orientation of the maximum principal strains were calculated from a 32 channel digital acquisition system. Results: For both specimens, the maximum principal strains at the maximum loading were highest in the medial wall (dome area) of the acetabulum. The tensile strain from the dome of the uncemented specimen at the maximum loading was twice that of the cemented specimen. In the cemented specimen, the compressive strains in the medial wall were almost twice the tensile strains at the maximum load. Within the acetabular quadrants, the highest strains were recorded in the posterio-inferior quadrant. Compressive strains in the posterio-inferior wall of the acetabulum seem to be comparable to those in the anterior-superior wall. Conclusion: The critical areas for load transfer in the acetabulum are the medial wall (dome area), the posterio-inferior and the anterior-superior quadrants. The uncemented cup appears to provide a better load transfer mechanism than the cemented cup

Long term clinical follow-up of total hip arthroplasty (THA) has identified problems associated with cyst formation. Such cysts are formed as a result of expansile osteolysis, which starts on a small area of the skeleton and spreads into the bone away from the surface of the prosthesis. Since large areas of the prosthesis are still in immediate contact with the skeleton the prosthesis is not loose and the patients are usually without pain. However this form of osteolysis may destroy large areas of the skeleton before it is detected and result in a sudden fracture due to a weakened skeleton. While there are some short term prospective trials that have shown changes in bone density in the periacetabular region, one needs a biomechanical model to understand factors that influence bone remodeling leading to cyst formation. This study aims to develop a mathematical model for studying the mechanical effects of bone cysts in the acetabulum of THA patients. 2D finite element (FE) models of patients with known restroacetabular cystic disease were generated using coronal CT images from the central region of the acetabulum. The boundary between bone and soft tissue was segmented and an FE model generated. Mesh convergence tests were performed to identify a suitable level of mesh refinement. Three material zones representing– cortical bone (E=17GPa), cancellous bone (E=1GPa) and a titanium cup (E=120GPa) – were included in the model. A series of simulations were run to investigate how cysts affect stress distribution as well as the mechanical consequence of medial wall deficiency. The presence of a cyst did not alter the pattern of stress distribution in the lateral and medial wall. But the strain energy function increased significantly at the inferior margin of the cyst within its cancellous bone. This may encourage bone formation at the cyst margin and help to explain the sclerotic walls seen in some cysts. Models with absent medial walls showed that both compressive and tensile stresses lowered in the cortical wall and the strain energy function reduced almost to zero. This suggests that a medial wall defect has a high risk of progression. The current 2D model cannot incorporate complex acetabular geometry or complex forces acting on the hip. Therefore the current model will be further developed into a 3D FE model of the whole pelvis that also represents the pelvic ring structure more adequately. Physiologically meaningful boundary conditions as well as patient specific geometry and material properties will be used to investigate mechanical effects of bone cysts realistically

Introduction: today there is still no consense in reconstruction of severe acetabular defects in hip revision. Since 1988 we use size matched impacted acetabulum allografts. We evaluate how they behave in the mid–and longterm. Materials and Methods: we present our first 44 transplants with a mean follow-up of 12,2 years (range 7 to 18). The mean age of the patients was 58,6 years (range 19 to 83). According to Gross Classification 26 cases presented an acetabular defect type III while 18 presented type IV. Evaluation included Merle D’Aubigne Score and radiological assessment of allograft and cup according to Engh Criteria (JBJS, 1994). Results: homogenization of the radiological trabecular pattern was observed in 42 from 44 cases (95,4%). There were 3 infections and 7 cases of aseptic loosening (15,9%), that were revised with new cup implantation. Up to now none of these 7 cases have shown further signs of loosening. According to the Kaplan-Meier’s curves the overall predictive survival rate was 76.4% at 15 years. In cases of pelvic discontinuity (type IV) survivorship was significantly higher (85,7%, p=0,018). There was a highly marked improvement in Merle D’Aubigne Score in Gait (2,2 preoperative – 4,9 at follow-up, p=0,021) and Pain (2,5 preoperative – 5,4 at follow-up, p=0,032). Conclusions: despite published reports with high incidence of failures in the midterm with structural allografts this serie shows clearly that a size matched impacted acetabulum allograft can be successfull in severe defficiencies if proper donor selection and excellent fixation technique is undertaken

Neglected fractures of the acetabulum have been defined as those fractures that present to the surgical team after 3 weeks from the time of injury. Total hip arthroplasty in these cases often require a major acetabular reconstruction. From 1989 until 2005, 38 total hip replacements were implanted in Cairo, Egypt for neglected acetabular fractures, with a follow up between 6 months and 192 months. The male to female distribution was 33/5. The age distribution ranged from 22 to 70 years with a mean value of 47.7 years. In 30 cases, hip arthroplasty was carried out as a primary procedure due to associated acetabular cartilage damage beyond reconstruction and/or associated articular cartilage or structural damage to the femoral head. 8 cases were done after a previous attempt of open reduction and internal fixation due to failure of the reduction and fixation, avascular necrosis and/or infection. 34 acetabular components were treated by internal fixation of the columns if still mobile, autografts from the femoral head with cemented acetabular components. Four uncemented acetabular components were used in selected cases when a stable reconstructed acetabular rim was achieved. All femoral stems were cemented with the exception of five cementless stems. At the last follow up, all grafted acetabular defects were taken with very satisfactory fixation in cemented cups. Three delayed acetabular loosening occurred at 13 years due to uncoupling of metal backed cemented acetabular components. Cementless acetabular components showed union of the fracture with stable acetabular fixation. The Merle D’Aubigne score was changed in all the cases from a range of 4/5 to 15/18. We believe that neglected acetabular fractures can be satisfactorily treated by cemented total hip replacement using internal fixation of the columns and acetabular grafting

Native anatomy of the arthritic hip is an important consideration in hip replacement surgery and implant design. Acetabular component orientation in total hip replacement (THR) is the single greatest factor that influences dislocation rate. Detailed knowledge regarding orientation of the native acetabulum is therefore essential. Native acetabular orientation in healthy hips is well documented but we could not find any papers detailing native acetabular orientation in the arthritic hip. A commercially available computer navigation system (Orthopilot BBraun Aesculap, Tuttlingen, Germany) was used to assess acetabular inclination and anteversion in 65 hips with symptomatic arthritis requiring THR. Acetabular inclination in all hips was also measured on pre op anteroposterior pelvic radiographs. Patients with DDH were excluded. All patients were Caucasian and had primary osteoarthritis, 29 males and 35 females. Average age 68(SD 8). Mean values as recorded by computer navigation were: inclination 51.4°(SD 7.1); anteversion 11.7°(SD 10.7). As recorded from radiographs mean acetabular inclination was 58.8°(SD 5.7). There was a difference between males and females. Mean navigated inclination: male 50.5°(SD 7.8); female 52.1°(SD 6.7). Mean navigated anteversion: male 8.3°(SD 8.7); female 14.39°(SD 11.6) Mean radiographic inclination: male 57.4°(SD 5.1) and female 59.8°(SD 6). Natural acetabular orientation in arthritic hips falls out with the safe zones defined by Lewinnek. When compared with healthy hips, as described in current literature, the arthritic hip appears to have a smaller angle of inclination and anteversion, by approximately 5° and 10° respectively, in both males and females. This is useful when positioning the cup during surgery. The difference between males and females, particularly in terms of anteversion, should also be considered

In the First Department of Orthopaedics of St. Anna University Hospital in Brno, Czech Republic, a total number of 9,393 total hip replacements (THR) were performed from 1964 to 2000. Since 1986, the cementless technique has been used primarily for young patients suffering from late DDH. Within the last ten years we have implanted 5,574 THRs, including 440 cementless systems and 2,122 hybrid THRs. We focused on cases of patients with late DDH where dysplastic acetabulum was found and where the smallest available cementless CLS acetabular cup was implanted through the anterolateral approach. We present five to seven years follow-up of 41 hips with dysplastic acetabulum that was evaluated by coxometry and where the expansion acetabular cup of diameter 46 mm was implanted together with different types of femoral stems. After five to seven years no significant aseptic loosening has been found. None of the Harris grafts of the superior acetabulum used in 10 (24.4%) implantations have failed. We have had good medium-term results with this type of cementless acetabular cup. A few case reports from our earlier history include acetabular components under 40 mm in diameter, or intercalar shortening osteotomy of the proximal femur due to the high post-dysplastic dislocation of the hip combined with implantation of a THR

Introduction and Objective

The patients with a total hip arthroplasty is growing in world manly in Europe and USA, and this solution present a high success at 10years in several orthopaedic registers. The application of total press-fit hip fixation presents the most used solution, but presents some failures associated to the acetabular component fixation, associated to the load transfer and bone loss at long term. The aim of this work is to investigate the influence of different acetabular bone loss in the strain distribution in iliac bone. To evaluate implant fixation, an experimental study was performed using acetabular press-fit component simulating different acetabular bone loss and measuring the strain distribution.

Materials and Methods

The experimental samples developed was based in an iliac bone model of Sawbones supplier and a acetabular component Titanium (Stryker) in a condition press-fit fixation and was implanted according surgical procedure with 45º inclination angle and 20º in the anteversion angle. Were developed five models with same initial bone, one with intact condition simulating the cartilage between bones and four with different bone loss around the acetabular component. These four models representing the evolution of bone support of acetabular components presented in the literature. The evolution of bone loss was imposed with a CAD CAM process in same iliac bone model. The models were instrumented with 5 rosettes in critical region at the cortical bone to measure the strain evolution along the process.

Introduction. Acetabular bone deficiency, especially proximal and lateral deficiency, is a difficult technical problem during primary total hip arthroplasty (THA) in developmental hip dysplasia (DDH). We report a configuration-based classification of hip, including a definition of shallow acetabulum. We also report a new reconstruction method using a medial reduced cemented socket and additional bulk bone in conjunction with impaction morselized bone grafting (Ad-BBG method). We aimed to evaluate usefulness of the classification and the method's clinical/radiographic outcomes. Methods. Forty percent of 330 THAs for DDH were defined as shallow dysplastic hips. The Ad-BBG method was performed on 102 hips (78% of shallow hips). For the 24 remaining hips, THA was performed using the conventional interposition bulk bone grafting (8 hips)or without bone grafting by using rigid lateral osteophyte (16 hips). Operative Technique: Theresected femoral head was sectioned at 1–2-cm thickness, and a suitable size of the bulk bone graft was placed on the lateral iliac cortex and fixed by polylactate absorbable screws. Autogenous impaction morselized bone grafting, with or without hydroxyapatite granules, was performed along with the implantation of medial reduced cemented socket. Radiographic criteria used for determining loosening were migration or a total radiolucent zone between the prosthesis/bone cement and host bone. The follow-up period was 10.2 ± 2.6 (range, 6.0–15.0) years. Results. Acetabular component was revised in only one case with a shallow and Crowe Type IV acetabulum. Within 2 years postoperatively, most Ad-BBGs cases showed successful bone remodeling and bone graft reorientation without collapse on radiographs. Discussion and Conclusions. Osteointegration and mid-term good clinical outcomes were achieved in acetabular reconstruction for primary THA using the medial reduced cemented socket and bone grafting methods including the Ad-BBG technique in conjunction with impaction morselized bone grafting for shallow dysplastic hip

There remains uncertainty about the most effective surgical approach in the treatment of complex fractures of the acetabulum. We have reviewed the experience of a single surgeon using the extended iliofemoral approach, as described by Letournel. A review of the database of such fractures identified 106 patients operated on using this approach with a minimum follow-up of two years. All data were collected prospectively. The fractures involved both columns in 64 (60%). Operation was undertaken in less than 21 days after injury in 71 patients (67%) and in 35 (33%) the procedure was carried out later than this. The reduction of the fracture was measured on plain radiographs taken after operation and defined as anatomical (0 to 1 mm of displacement); imperfect (2 to 3 mm) or poor (> 3 mm). The functional outcome was measured by the modified Merle d’Aubigné and Postel score. The mean follow-up was for 6.3 years (2 to 17). All patients achieved union of the fractures. The reduction was graded as anatomical in 76 (72%) of the patients, imperfect in 23 (22%), and poor in six (6%). The mean Merle d’Aubigné and Postel score was 15 (5 to 18) with 68 patients (64%) showing good or excellent and 38 (36%) fair or poor results. Function correlated significantly with the accuracy of the reduction (p < 0.009). Significant heterotopic ossification developed in 32 patients (30%) and was associated with a worse mean Merle d’Aubigné and Postel score of 13.7. The extended iliofemoral approach can be performed safely in selected complex acetabular fractures with an acceptable clinical outcome and rate of complications. Effective prophylaxis against heterotopic ossification should be strongly considered

Purpose: The purpose of this study was to assess outcome after first-intention total hip arthroplasty for fresh fractures of the acetabulum in elderly patients. Material and methods: We present a retrospective analysis of 18 recent fractures of the acetabulum observed in nine men and nine women, mean age 74.8 years which were treated by first-intention total hip arthroplasty. The fractures were considered equivalent to acetabular bone deficits observed at revision of total arthroplasty (five grade III, 13 grade IV in the SOFCT 1988 classification). The prosthesis was implanted 15.6 days (mean) after initial trauma and included a metal-backed acetabular implant (except one case) and a bone autograft (except three cases). Complete weight bearing was authorised on day 3 postop for five patients, and at six weeks for ten; at three months for the other three. Mean follow-up was 2.5 years. Thirteen surviving patients were reviewed clinically and radiographically. Five patients who had died were also included in the analysis using data reported by family before death. The Postel-Merle d’Aubigné (PMA) and Harris scores were recorded. Survival curves were plotted. Radiological assessment included bone healing, status of the prosthetic assembly, presence of a lucent line or signs of loosening in the three acetabular zones described by De Lee and Charnley. Results: The mean functional scores were 13.6/18 (PMA) and 71.8/100 (Harris). There were no cases of infection or dislocation. Median Kaplan-Meier survival was six years. The one-year survival rate was 94.4%. Five deaths were recorded, all due to causes independent of the surgical intervention. Radiologically, all fractures had healed. There were no mechanical complications and no signs of acetabular loosening. Discussion: Several authors have demonstrated that prognosis is poor in elderly subjects undergoing surgical osteosynthesis or functional treatment due to the presence of risk factors (osteoporosis, comminution, deferred surgery due to poor general status). Prolonged bed rest may also have life-threatening consequences. Other work has emphasised the very high rate of complications after revision surgery for dismounted material, post-traumatic deterioration, or osteonecrosis. The absence of early deaths and the low rate of intervention-related morbidity in this series of elderly patients is in agreement with a very small number of publications devoted to this topic. Conclusion: This therapeutic attitude enables early verticalisation avoiding the complications related to the bedridden state in the elderly. In addition, subsequent revision for osteonecrosis of the femoral head or post-traumatic deterioration can be avoided. These results have incited us to pursue this therapeutic option

Radiographs of 155 Indian children were examined to identify the acetabular changes which occur in Perthes' disease. These changes included osteoporosis of the acetabular roof, irregularity of contour, premature fusion of the triradiate cartilage, hypertrophy of articular cartilage and changes in dimensions. These changes tended to be more marked in older children and when more than half of the femoral epiphysis was involved. Comparison with 25 cases of Perthes' disease from Liverpool showed the same picture. Several of the acetabular changes noted during the active stages were also seen in a series of 24 adult hips after Perthes' disease. Radio-isotope scans of the hips of 27 children with Perthes' disease showed a consistently increased uptake in the acetabulum on the affected side, indicative of a local increase in vascularity and metabolic activity. It was possible to postulate a working model for the pathogenesis of all the acetabular changes. A number of statistical correlations suggest that most of the changes have a bearing on the final outcome


Objective. The optimal positioning of the acetabular component is a relevant prognostic factor in total hip arthroplasty (THA). Because of substantial errors of manual technique in cup placement even with experienced surgeon, computer aided navigation system has been developed in recent years. However, existence of the hardware around acetabulum likely deteriorates the accuracy of the navigation system, namely in revision THA case and postoperative status of pelvic fracture. Here we report a case who we successfully performed THA using CT based navigation system although there were multiple hardware around acetabulum due to osteosynthesis for the previous pelvic fracture. Case presentation. A forty-one years old man presented with intolerable hip pain with severe radiographic osteoarthritic findings in left hip joint. He had sustained left pelvic fracture and posterior hip dislocation due to traffic accident and undergone osteosynthesis using multiple plates and screws when he was forty years old. However, progressive collapsing of femoral head and acetabulum occurred. Then, we indicated THA for his situation and planned to apply the CT based navigation system (Stryker CT based hip Ver.1.1 softwear and Cart II system). Preoperative workup revealed incomplete union of posterior and superior acetabular wall and we had to retain plates and screws for the stable fixation of acetabular cup. The existence of the hardware made it complicated to perform three dimensional planning and templating. Meticulous surface editing of pelvis to exclude the metal artifact and fibrocartilagenous tissue was needed to achieve accurate surface registration. In the operation room, we had to use unusual way of registration to complete two steps of registration. In the first step (roughly matching between patient's physical pelvic surface and edited pelvic surface in work station using corresponding 5 points), we utilized head of screw and hole of the plate which we could easily identify intraoperatively, in addition to ASIS and innominate groove. In the second step (strict matching using more than 30 points of pelvic surface), we had to identify the pelvic bony surface, as excluding the metal surface and fibrocartilagenous tissue such as fracture callus. These efforts enabled us to accomplish substantial accuracy of registration with RMS of 0.5 mm. Final cup orientation at the end of surgery was 41° of inclination and 25° of anteversion. Postoperative CT scan revealed that cup placement angle was 40° of inclination and 25° of anteversion, almost identical with intraoperative value. Conclusion. Our experience showed that CT based navigation system provided accurate placement of the acetabular component in a case having multiple hardware as well as in normal primary THA. Although we need additional efforts such as meticulous preoperative planning, extra operation time, CT based navigation system has great advantages to minimize the mal-placement of the cup in complicated case

Introduction: Pressurization of PMMA can lead to cement extrusion. Although rare, the complications related to cement extrusion (‘CE’) are serious such as neurological, urological and vascular injuries. In a recent study the incidence of CE from acetabulum was found to be 42–50%, most of which was under transverse ligament. We advocate a technique of applying cancellous autograft on the medial floor and under the transverse ligament to provide cancellous surface for cement pressurisation, and to prevent CE. The aim of this study was to review our incidence of CE and radiolucent lines (‘RLL’). Methods: Study group included 380 consecutive patients undergoing primary implantation of flanged cemented cup with spacers during 2002–2003. The mean age was 68 years. Surgeons of all grades, including junior doctors under supervision, performed the procedure. Early postoperative radiographs were reviewed to identify the incidence, site and extent of CE and incidence of RLL. Results: We identified CE in 46 radiographs, 35 being inferior, 6 pelvic and 5 along retractors. The mean size of the CE was 240.5 mm. 2. Radiolucent lines (RLL) in any of the Charnley zone were present in 58 cases. Two zone RLL were seen in seven cases (1.8%) and a circumferential radiolucency in one case (0.3%). There was no significant correlation between the grade of the operating surgeon and the incidence of CE (p, 0.15). There was no significant correlation between the grade of operating surgeon and the presence of RLL (p, 0.18). Discussion: Results of this study confirm that incidence of CE with this technique is significantly less as compared with historic data (12% vs. 42%). Incidence of RLL is also less especially in zone 2 and 3. These findings support our hypothesis that use of autograft to convert acetabulum in contained hemisphere reduces incidence of CE and improves pressurization

Introduction: Osseointegration is known to occur around the uncemented acetabular cups which results in fill-in of peri-acetabular gaps. The objective of this study was to assess the gaps around uncemented acetabular cups radiologically in early post-operative period. Methods: 53 primary uncemented total hip arthroplasties were performed at our hospital by a single surgeon between February 2003 and august 2005. There were 29 females and 22 males. Two patients had bilateral surgeries. Mean age of patients was 70 years (range, 52–88 years). Primary osteoarthritis of the hip was the indication for surgery in all the patients. Peri-acetabular gaps were measured on the radiographs taken at day-1 post-operatively and at 3 months follow-up. All the measurements were taken independently by two investigators on two different occasions using a picture archiving & communications (PACS) system. The two sets of data from each investigator were compared for intra and inter-observer variability using independent-samples t test. Results: in 24 cases, no gaps were found around the ace-tabular cups on day-1 post-operative radiographs. In 29 cases, the mean gap was 4 mm (range, 1–8 mm) on postoperative day-1. Five gaps were in zone one, 24 in zone two and none in zone three. At 3 months follow-up, the mean gap was found to be 0.6 mm (range, 0–3 mm). The reduction in gaps from day-1 post-op to 3 months follow-up was statistically significant (chi square test, p< 0.05). Discussion: We found a significant reduction in peri-acetabular gaps as early as at 3 months following uncemented total hip arthroplasties. We feel that settling of the cup within the acetabulum is responsible, rather than osseointegration, for these fill-in of gaps in early postoperative period. A larger study is required to analyse this phenomenon as screws fixation of uncemented cups may compromise their settling within the acetabulum

We aimed to identify variables associated with clinical and radiological outcome following fractures of the acetabulum associated with posterior dislocation of the hip. Using a prospective database of 1076 such fractures, we identified 109 patients with this combined injury managed operatively within three weeks and followed up for two or more years. The patients had a mean age of 42 years (15 to 79), 78 (72%) were male, and 84 (77%) had been involved in motor vehicle accidents. Using multivariate analysis the quality of reduction of the fracture was identified as the only significant predictor of radiological grade, clinical function and the development of post-traumatic arthritis (p < 0.001). All patients lacking anatomical reduction developed arthritis whereas only 25.5% (24 patients) with an anatomical reduction did so (p = 0.05). The quality of the reduction of the fracture is the most important variable in forecasting the outcome for patients with this injury. The interval to reduction of the dislocation of the hip may be less important than previously described

The results of using the Ring uncemented polyethylene-to-metal (UPM) prosthesis in patients with protrusio acetabuli or dysplasia are reported. Fifty-four cases of protrusio were analysed and a modified method of measuring the deformity is described; after operation, remodelling of the medial wall had improved, on average, by 29.6%. For the dysplastic acetabulum the main difficulty is the lack of lateral support for the cup. In order to test the hypothesis that this support improves with an uncemented prosthesis, 38 hips with an average follow-up of 31 months were studied. Lateral support, measured radiographically, showed an average increase of 21%. The remodelling effect may be attributed to the biomechanical design of the prosthesis

Our aim in this prospective study was to compare the bone mineral density (BMD) around cementless acetabular and femoral components which were identical in geometry and had the same alumina modular femoral head, but differed in regard to the material of the acetabular liners (alumina ceramic or polyethylene) in 50 patients (100 hips) who had undergone bilateral simultaneous primary total hip replacement. Dual energy X-ray absorptiometry scans of the pelvis and proximal femur were obtained at one week, at one year, and annually thereafter during the five-year period of the study. At the final follow-up, the mean BMD had increased significantly in each group in acetabular zone I of DeLee and Charnley (20% (15% to 26%), p = 0.003), but had decreased in acetabular zone II (24% (18% to 36%) in the alumina group and 25% (17% to 31%) in the polyethylene group, p = 0.001). There was an increase in the mean BMD in zone III of 2% (0.8% to 3.2%) in the alumina group and 1% (0.6% to 2.2%) in the polyethylene group (p = 0.315). There was a decrease in the mean BMD in the calcar region (femoral zone 7) of 15% (8% to 24%) in the alumina group and 14% (6% to 23%) in the polyethylene group (p < 0.001). The mean bone loss in femoral zone 1 of Gruen et al was 2% (1.1% to 3.1%) in the alumina group and 3% (1.3% to 4.3%) in the polyethylene group (p = 0.03), and in femoral zone 6, the mean bone loss was 15% (9% to 27%) in the alumina group and 14% (11% to 29%) in the polyethylene group compared with baseline values. There was an increase in the mean BMD on the final scans in femoral zones 2 (p = 0.04), 3 (p = 0.04), 4 (p = 0.12) and 5 (p = 0.049) in both groups. There was thus no significant difference in the bone remodelling of the acetabulum and femur five years after total hip replacement in those two groups where the only difference was in the acetabular liner

The bone-marrow oedema syndrome is associated with local vascular disturbances and may be treated either conservatively or by core decompression after which recovery may take several weeks. We describe a 15-year-old girl with bone-marrow oedema of the left acetabulum which was confirmed by MRI. She presented with a four-week history of severe constant pain. Routine blood tests and plain radiographs were normal. She was treated with intravenous infusions of iloprost on five consecutive days (20 μg administered in 500 ml of sodium chloride). Iloprost causes vasodilatation with reduction of capillary permeability and it inhibits platelet aggregation. She had relief from pain at rest after three days of treatment and was completely free from symptoms after two weeks. MRI after six weeks showed almost complete resolution of the marrow oedema and was normal after four months. This is the first report of the pharmacological treatment of the bone-marrow oedema syndrome in children

We describe a patient in whom an initially intact sciatic nerve became rapidly encased in heterotopic bone formed in the abductor compartment after reconstruction of the posterior wall of the acetabulum following fracture. Prompt excision and neural release followed by irradiation and administration of indometacin resulted in a full neurological recovery and no recurrence 27 months later

Salter’s innominate osteotomy predisposes the hip to acetabular retroversion as it hinges upon the symphysis pubis. Retroversion is a recognised cause of osteoarthritis, hip pain and clinical signs of impingement, but there is uncertainty as to whether this over cover persists with growth and development. We reviewed the long-term follow up of twenty patients that had undergone a Salter’s osteotomy between 1985 to 1993 at The Royal Orthopaedic Hospital Birmingham or New Cross Hospital Wolverhampton. Sixteen skeletally mature patients were available for review that had previously had the pelvic osteotomy performed at a mean five years of age with a contralateral normal hip. Salter’s osteotomy had been performed for developmental dysplasia of the hip in 13 patients and for Perthes’ disease in three patients. Follow up was performed at an average age of 20 years. Outcome was assessed using the Harris Hip Score and a clinical examination for signs of impingement and by a measurement of acetabular version, on well centered pelvic radiograph. Acetabular version was evaluated by the relationship between anterior and posterior walls of both the normal and Salter acetabulum, using radiographic templates as described by Hefti. Mean acetabular version averaged 16.9 degrees (95% CI 7.6 to 26.1) of anteversion on the Salter side and 17.6 degrees (95% CI 10.4 to 24.8) anteversion on the contralateral normal hip. There was no statistical difference between the version on operated and normal hips, paired t test (p = 0.83). Harris Hip Score averaged 85, indicating a good outcome at long-term follow up. Two patients (12%) demonstrated retroversion, however neither of these had signs of impingement on clinical examination. After a Salter innominate osteotomy in childhood, we believe there is remodelling of acetabular version by skeletal maturity

The purpose of current study was to describe the results of complex acetabular fractures treated with open reduction using transtrochanteric approach and arthrotomy of the hip joint. Fourteen consecutive patients with both column fractures of the acetabulum were treated with open reduction and internal fixation. All patients had various associated injuries. Among them, one patient had pelvic abscess associated with traumatic bowel perforation. The acetabulum was approached with Y-shaped triradiate incision, osteotomy of the greater trochanter, and arthrotomy of the hip joint. During the operation, the osteochondral fragments were removed and torn labrum was resected. In 6 patients the fracture was fixed with reconstruction plates and in 8 patients the fracture was fixed with plates and wires. All the patients were followed for an average of 4.6 years(range, 2–8 years). The clinical evaluation was done by the method of Merle d’Aubigne. All the fractures and all osteotomies united at the latest follow up. One patient had delayed hematogenous infection at 5.5 years after the operation. Although myositis ossificans developed in 3 patients it was neither progressive after 1 year nor associated with significant limitation of hip motion. Four patients had narrowing of the hip joint space. Three of them had osteophyte formation around the femoral head. No femoral head necrosis was observed. Eleven patients had excellent or good outcomes in clinical score. No patient underwent total hip arthroplasty. This extensile approach allowed a good exposure of the fracture site, more accurate reduction, and easier fixation of fracture fragments. It also allowed the removal of osteochondral fragments and the resection of torn labrum. However, 3 patients showed osteophyte formation around the femoral head. We are concerned about the further progression of the osteophyte and its clinical implication

Introduction: This study was initiated to evaluate cortical and cancellous bone density (BD) changes of the acetabulum after cemented and uncemented total hip arthroplasty (THA) using computer tomography (CT)-assisted osteodensitometry in-vivo. Materials and Methods: 15 cemented ZCA Pfannen cups (Zimmer, USA) (age 78 years) and 21 press-fit Trilogy cups (Zimmer, USA) (age 72 years) were implanted by one surgeon. All hips were investigated by a standardized CT-mode (slice thickness 2 mm, table feed 5 mm, extended CT-scale). 6 CT-scans at the level of the cup and 4 scans above the dome of the cup were analyzed 2 weeks and 2 years after surgery. Cancellous and cortical bone mineral density (BMD) (CaHA mg/ml) were elaborated with a special software tool (CAPPA postOP, CAS Innovations AG, Erlangen). Results: 2 years after index operation cemented cups showed mild cancellous BD loss (−8% to −20%) and no significant cortical BD changes cranial, significant cortical (−4% to −20%) and cancellous (−16% to −44%) BD loss ventral and no significant BD changes dorsal to the cup. For press-fit cups we observed highly significant (p< 0,01) cancellous BD loss in all sectors (−17% to −53%), cortical BD loss ventral and dorsal to the cup (−12% to −23%) and very limited BD loss cranial (−4% to −13%) to the cup. Conclusions CT-assisted osteodensitometry allows a thorough assessment of the actabular bone in-vivo. Different patterns of stress shielding were observed for cemented and press-fit cups. For the press-fit cup high BD loss for both cortical and cancellous bone was observed in all areas adjacent to the pelvic implant, except for cortical BD at the acetabular dome, suggesting fixation of the cup in the cranial cortical bone. Comparetively less BD loss was seen for cemented cups in all sectors, especially for cancellous bone, suggesting a more physiological stress transfer to both cortical and cancellous pelvic bone

Introduction. The clinical and functional outcome following simultaneous fractures of the acetabulum and pelvic ring represent a challenge for the clinician and are currently obscure in the existing literature. Patients and Methods. Between January 2003 and July 2009 all patients with combined pelvic and acetabular fractures were included in this prospectively study. Demographic details, mechanism of injury, ISS, surgical procedures, post-operative course, length of hospital stay, and radiological (Matta's score) and functional outcomes (The Merle d' Aubigne hip score) were recorded. The minimum follow up was 12 months (12–72). Results. Out of 520 cases, 30 (21 males) patients met the inclusion criteria. The mean age was 32. 55% sustained lateral compression, 40% combined mechanism and 5% anterior/posterior type of injury pattern. 15 out of the 30 patients had sustained other injuries. The mean time from injury to definite reconstruction was 7 days (3–14). Ilioinguinal approach was used in 20 patients, Pfannestiel in 2 and Kocher-Langebeck in 4. A wheelchair was used initially in 22 cases. The median time to FWB was 3 months. One patient underwent a THR at 18 months. 19 patients scored the full 18 points of the Merle score. Matta radiological score was excellent in 16 patients, Good 11, Fair 2, Poor 1. Conclusion. The management of these injuries poses many difficulties to the trauma surgeon. With the available expertise in place good outcomes can be obtained as seen in this series of patients

Mal-positioning of the acetabular component is associated with increased dislocation rate, increased wear and component impingement. Navigation provides real time feedback to the surgeon and allows the accurate position of implants. Compared to conventional techniques of total hip replacement; use of the imageless navigation system has shown to improve accuracy of implant positioning. When impacting uncemented acetabular components under navigation, there is often a deviation from the planned abduction and anteversion measurement due to deflection of the implant in the reamed cavity. Although there exists the ability to navigate the reaming of the acetabular cavity; this is not widely performed. The ability to ream the acetabular cavity in the exact orientation of the planned acetabular component may provide some theoretical advantage on the final acetabular position. The purpose of this study was to compare the effect of navigated Vs free hand acetabulum reaming on achieving the planned orientation of acetabular component. In a retrospective study we reviewed two groups of patients who underwent computer navigated placement of the acetabular component with reference to the anterior pelvic plane. We used an imageless computer navigation system for all cases (Brainlab, Munich). All procedures were performed by single surgeon (ETD) through a standard posterior approach. The patients were divided into two groups depending on the availability of the navigated reamer. In the first group (n = 57), acetabulum reaming was done under navigation and in the second group (n = 37) a non-navigated reamer was used. The acetabular cavity was reamed “line to line” or under reamed by 1 or 2mm. Intra-operative acetabular abduction and anteversion angles were planned using navigation at the discretion of the surgeon. Results of planned acetabular abduction and anteversion angles were compared with intra-operative verification using the navigation system. In the navigated reamer group, the mean error from the planned to verified abduction angle was 1.7 degrees (SD 2.1 degrees) and in the non-navigated reamer group the mean error was 2 degrees (SD 2.6 degrees). In the navigated reamer group, the mean error from the planned to verified anteversion angle was 0.5 degrees (SD 2.8), and in the non-navigated reamer group the mean error was 0.1 degrees (SD 1.6). There was no statistically significant difference in the mean error between the navigated and non-navigated reaming groups for abduction angle (p = 0.54) or anteversion angle (p = 0.24). There was no statistical difference between the mean acetabular component size in the navigated (mean 53mm) and non-navigated (53mm) reamer groups (p = 0.8). There was no statistical difference in the mean difference in reamer size and the acetabular component size in the navigated (0.8mm) and non-navigated reamer groups (0.8mm, p = 0.52). This study appears to show that performing reaming of the acetabular cavity under navigation does not improve the final orientation of the acetabular component when compared to using conventional non-navigated reamers. However, this study only considered the abduction and anteversion orientation of the component. The move to a range of movement or kinematic orientation of the acetabular component in hip arthroplasty requires control over the off-set of the acetabular component which may be more easily achieved when the reaming is performed under navigation. This study used a conventional posterior approach rather than a minimal incision technique, where the use of navigated reaming may also provide some theoretical advantage when visibility is limited. Further study is required in these two areas. There appears to be a slightly higher standard deviation for the anteversion measurement in the navigated reamer group when compared to the non navigated reamer group, although this is not significant. It is difficult to account for this as it appears to be opposite of what one would predict. One explanation for this may come in the difference in the angled geometry of the navigated reamer when compared to the straight non navigated reamer. The angled reamer can be more difficult to control forming a cavity in the correct orientation but with the possibility for the cavity to not been perfectly hemispherical. When using navigation to insert the acetabular component in a planned abduction and anteversion position during hip arthroplasty through a standard incision, navigating the reaming of the acetabular component does not appear to provide any advantage over the use of conventional non-navigated reamers in the final acetabular orientation

We studied cartilage degeneration in 45 canine acetabula after implantation of prostheses with articulating surfaces of low-temperature isotropic (LTI) pyrolytic carbon, cobalt-chromium-molybdenum alloy and titanium alloy for periods ranging from two weeks to 18 months. Gross specimens and histological sections were compared with the nonoperated (control) acetabulum of the same animal. Cartilage articulating with LTI pyrolytic carbon exhibited significantly lower levels of gross wear, fibrillation, eburnation, glycosaminoglycan loss, and subchondral bone change than with metallic surfaces. Survivorship analysis showed a 92% probability of survival for cartilage articulating with LTI pyrolytic carbon at 18 months, as compared to only a 20% probability of survival for cartilage articulating with either of the metallic alloys

Aims

Studies of infant hip development to date have been limited by considering only the changes in appearance of a single ultrasound slice (Graf’s standard plane). We used 3D ultrasound (3DUS) to establish maturation curves of normal infant hip development, quantifying variation by age, sex, side, and anteroposterior location in the hip.

Methods

We analyzed 3DUS scans of 519 infants (mean age 64 days (6 to 111 days)) presenting at a tertiary children’s hospital for suspicion of developmental dysplasia of the hip (DDH). Hips that did not require ultrasound follow-up or treatment were classified as ‘typically developing’. We calculated traditional DDH indices like α angle (αSP), femoral head coverage (FHCSP), and several novel indices from 3DUS like the acetabular contact angle (ACA) and osculating circle radius (OCR) using custom software.

Introduction. Optimal alignment of the acetabulum cup component is crucial for good outcome of Total Hip Arthroplasty (THA). A patient-specific instrumentation (PSI) for cup alignment manufactured by 3D printing might improve cup alignment in conventional THAs with patient's lateral decubitus position. In this study, we developed PSI for cup alignment which transferred preoperatively planned cup alignment to the operation room as a linear visual reference(Figure 1), then investigated its accuracy in terms of fitting of PSI on the bony surface and angle deviation between pre- and post-operative cup alignments. Methods. 3-Dimensional bone models created from CT images of both sides of 6 cadaveric specimens were used in the current study. In the first experiment (first 3 specimens and six hips), we designed PSI to fit on the acetabular rim, and we inserted a Kirschner wire (K-wire) through PSI after PSI's fitting. In the second experiment (remaining 3 specimens and six hips), after the same steps like the first experiment were done, we reamed and finally impacted plastic cups with the visual reference of the K-wire. Using postoperative CT images taken after both experiments, we measured deviation of the K-wire placement for the first experiment, and measured deviation of the cup placement from planned cup alignment. Results. The angle deviation of the K-wire alignment on the basis of radiographic inclination and anteversion angles was on average 2.2°±2.5° and 1.0°±1.3° respectively in the first experiment. The angle deviation of the cup alignment with the same definition was on average 2.88°±1.63° and 4.15°±2.56°. For one cadaveric specimen data for the first experiment were missing. Conclusion. We conclude that the accuracy of acetabular cup placement can be improved by the use of patient-specific cup orientation guides

Aims: The purpose of this study was to quantify the amount of cell viability and cartilaginous damage present in non-reparable human osteoarticular fragments removed at the time of acetabular fracture surgery. Material and Methods: The cases of 6 patients with comminuted fractures of the acetabulum were prospectively analyzed. Average age was 39 years, and none of them had evidence of preexisting hip pathology. Loose small osteoarticular fragments that were not reparable were microscopically analyzed to assess in-situ cell viability. Observations were divided into (i) depth of chondrocyte death from the articular surface, and (ii) structural matrix damage and cell death under regular histology. The depth of cell death was classiþed as mild between 1 and 15%, moderate from 15 to 30%, severe from 31 to 60% and total from 61 to 100%. Results: Five of the patients were classiþed as having only mild amount of chondrocyte death and one specimen had a moderate amount of chondrocyte death. The articular surface damage was mainly located on the superþcial zone of the cartilage. Discussion and conclusion: Most of the chondrocytes on small osteochondral fragments removed from displaced intraarticular acetabular fractures were still viable after having received a substantial amount of trauma

In Revision-THR the great variability of acetabular defects requires a revision-cup-system, which enables the surgeon to treat even extensive unexspected bone losses with a load stable reconstruction during the surgery. For these cases a modular revision support cup (MRS-Titan) has been developed. It allows the reconstruction of the geometric rotation centre and prevents the applied autologous or homologous bone graft in the healing phase from overlaoding. Beyond that an individual adjustment of the differently large flexible straps guarantees the solid anchoring of the revision support cup to the vital bone. The individual anatomy can be preserved in every case of acetabular destruction due to the high range of modularity the system provides. Because of the intraoperatively synthesis of all parts of the MRS-cup the approach and the traumatization of the soft tissue can be minimized. Since 1995 we implanted 95 MRS-Titan-cups out of 250 which were implanted world wide. In all cases a stable anchoring of the implant has been reached. We will report about our own follow-up and complications. In 4 MRS revisions a revitalized acetabular bone graft has been found able to host a noncemented hemispherical cup in three cases. Conclusion: The MRS-System is able to bridge mechanically stable damaged or missing parts of the acetabulum and allows immediate partial loading. A solid bone remodeling can be achieved. The costs could be minimized by reduction of store-keeping and simple but well considered instruments

Aims

Manual impaction, with a mallet and introducer, remains the standard method of installing cementless acetabular cups during total hip arthroplasty (THA). This study aims to quantify the accuracy and precision of manual impaction strikes during the seating of an acetabular component. This understanding aims to help improve impaction surgical techniques and inform the development of future technologies.

Methods

Posterior approach THAs were carried out on three cadavers by an expert orthopaedic surgeon. An instrumented mallet and introducer were used to insert cementless acetabular cups. The motion of the mallet, relative to the introducer, was analyzed for a total of 110 strikes split into low-, medium-, and high-effort strikes. Three parameters were extracted from these data: strike vector, strike offset, and mallet face alignment.

Aims

The aim of the study was to investigate whether the primary stability of press-fit acetabular components can be improved by altering the impaction procedure.

Methods

Three impaction procedures were used to implant acetabular components into human cadaveric acetabula using a powered impaction device. An impaction frequency of 1 Hz until complete component seating served as reference. Overimpaction was simulated by adding ten strokes after complete component seating. High-frequency implantation was performed at 6 Hz. The lever-out moment of the acetabular components was used as measure for primary stability. Permanent bone deformation was assessed by comparison of double micro-CT (µCT) measurements before and after impaction. Acetabular component deformation and impaction forces were recorded, and the extent of bone-implant contact was determined from 3D laser scans.

Background: We examined the role of the geometry of acetabulum in the development of osteoarthritis in young men. Contrary to previous studies we evaluated the significance of the anterior, posterior and total coverage of the femoral head, the influence of the femoral neck and the consequence of acetabular retroversion on standardized x-rays. Material and Methods: Men (n=78) aged 26–55 years (case-group) were selected from the Danish Hip Arthroplasty Register. This group was compared to a control-group (n=78) selected from men of the same age group received as trauma patients. Anterior/posterior pelvic radiographs were evaluated taking into account the degree of pelvic rotation and tilt. Results: Femoral head coverage, femoral head-neck ratio and centre edge angle were significantly smaller in the case-group, acetabular index angle and modified ACM-angle were significantly bigger. Anterior, posterior and total coverage of femoral head showed no difference. The frequency of acetabular retroversion was greater among cases. Interpretation: Osteoarthritis of young men is associated with a small superior acetabular coverage, a steep acetabular roof, a small acetabular depth, a thickened femoral neck and acetabular retroversion

We describe a 15-year-old boy with a posterior dislocation of the hip, fracture of the posterior column of the acetabulum and separation of the femoral capital epiphysis. To our knowledge no previous case in a child has been reported. Such high-energy injuries are extremely rare, and a poor outcome is expected. We advocate early referral to a specialised tertiary centre, and the use of a modification of Delbet’s classification to reflect the complexity and displacement which may occur with this injury

Introduction: today there is still no consense in reconstruction of severe acetabular defects in hip revision. Since 1988 we use size matched impacted acetabulum allografts. We evaluate how they behave in the mid- and longterm. Material and method: we present our first 44 transplants with a mean follow-up of 11,2 years (range 6 to 17). The mean age of the patients was 58,6 years (range 19 to 83). According to Gross Classification 26 cases presented an acetabular defect type III while 18 presented type IV. Evaluation included Merle D’Aubigne Score and radiological assessment of allograft and cup according to Engh Criteria (JBJS, 1994). Results: homogenization of the radiological trabecular pattern was observed in 42 from 44 cases (95,4 %). There were 3 infections and 7 cases of aseptic loosening (15,9 %), that were revised with new cup implantation. Up to now none of these 7 cases have shown further signs of loosening. According to the Kaplan-Meier’s curves the overall predictive survival rate was 76.4 % at 15 years. In cases of pelvic discontinuity (type IV) survivorship was significantly higher (85,7%, p=0,018). There was a highly marked improvement in Merle D’Aubigne Score in Gait (2,2 preoperative – 4,9 at follow-up, p=0,021) and Pain (2,5 preoperative 5,4 at follow-up, p=0,032). Conclusions: despite published reports with high incidence of failures in the midterm with structural allografts this serie shows clearly that a size matched impacted acetabulum allograft can be successfull in severe defficiencies if proper donor selection and excellent fixation technique is undertaken

Post-traumatic periprosthetic acetabular fractures are rare but serious. Few studies carried out on small cohorts have reported them in the literature. The aim of this work is to describe the specific characteristics of post-traumatic periprosthetic acetabular fractures, and the outcome of their surgical treatment in terms of function and complications.

Methods

Patients with this type of fracture were identified retrospectively over a period of six years (January 2016 to December 2021). The following data were collected: demographic characteristics, date of insertion of the prosthesis, details of the intervention, date of the trauma, characteristics of the fracture, and type of treatment. Functional results were assessed with the Harris Hip Score (HHS). Data concerning complications of treatment were collected.


Introduction: This paper describes the results of treatment of THA in irradiated hips, and analyzes the risk factors related to the early failure of the acetabular components. Materials and Methods: Eighteen primary total hip arthroplasties (twelve patients) were analyzed. The mean follow-up period was fifty-eight months (range, twenty to 139 months). The acetabular components were of hemispherical design with an ingrowth surface of titanium fiber-metal in fourteen cases and roof rings in four cases. Risk factors were analyzed between age, total radiation dose, onset of symptoms after irradiation (≤ 4 years, > 4 years), and the risk of failure in the acetabular components after THA. Results: When only nine cases of failure were considered, the interval from operation to failure had a positive association with total radiation dose and a negative association with the ages of the affected patients or the latent period before onset of symptoms. When all the cases were included in the survival analyses, the survival rates did not depend on either patient age or total radiation dose. The latent period was significantly related to the survival rate with a cutoff point of four years (p = 0.03). After age, total radiation dose, and component type were adjusted, the risk of failure was fifteen times higher (95% confidence interval: 1.03–227.44) in patients ≥ 4 years of the latent period, compared to those < 4 years of the latent period. Conclusions: It is suggested that any arthroplasty other than a resection arthroplasty is unwise if the portal of irradiation includes the acetabulum and the latent period of the disease onset after irradiation is more than four years

Aims

This study aimed to evaluate the accuracy of implant placement with robotic-arm assisted total hip arthroplasty (THA) in patients with developmental dysplasia of the hip (DDH).

Methods

The study analyzed a consecutive series of 69 patients who underwent robotic-arm assisted THA between September 2018 and December 2019. Of these, 30 patients had DDH and were classified according to the Crowe type. Acetabular component alignment and 3D positions were measured using pre- and postoperative CT data. The absolute differences of cup alignment and 3D position were compared between DDH and non-DDH patients. Moreover, these differences were analyzed in relation to the severity of DDH. The discrepancy of leg length and combined offset compared with contralateral hip were measured.

We aimed to investigate the clinical consequences of intraoperative acetabular fractures. Between 2003 and 2012, a total of 3391 cementless total hip arthroplasties (THA) were performed at the Dept. of Orthopaedics, Innsbruck Medical University. Of those, a total of 160 patients underwent a CT scan within 30 days postoperatively. The scans of 44 patients were not suitable for analyse due to thick scan layers. Of the remaining 116 patients, 76 had a fracture. Reasons for CT-scans were suspected bleeding, hip pain, abdominal symptoms, etc.

The fractures included 59 isolated acetabular fractures, the rest of fractures was in the superior or inferior pubic ramus or the tuber ischiadicum.

Four cases out of the 59 acetabular fractures underwent revision surgery due to periprosthetic joint infection after 0, 2, 10 and 23 months. Four patients underwent revision due to cup loosening after 13 and 14 days as well as after 16 and 24 months. Of those, three showed a central acetabular fracture with protrusion.

In 33 of the remaining 51 patients, a minimum of 3 x-rays was available for migration analysis with EBRA. In 6 patients, the x-rays were not comparable to each other. The 27 remaining acetabular fractures were categorised according to AO classification in 62A1 (1; posterior wall), 62A2 (16; posterior column), 62A3 (6; anterior wall), and others (4). Four hips showed initial migration of more than 3 mm in the first 6 months. One had a central fracture, and was lost for follow-up after 8 months. Two had an anterior column fracture and showed no further migration after 6 months. One showed also a radiolucency of more than 2 mm in all 3 zones and was lost for follow-up.

We conclude that intraoperative acetabular fractures occur more often than we expected. Fractures of the acetabular ring involving one column do not seem to compromise the long-term stability of the implant. Central fractures required revision or showed loosening proved by high cup migration.

Aseptic loosening of the acetabular component continues to be the most common indication for revision of total hip replacements in younger patients. Early in the evolution of the cemented hip, arthroplasty surgeons switched from removal to retention of the acetabular subchondral bone plate, theorising that unfavourable mechanical forces were the cause of loosening at the bone-cement interface.

It is now known that the cause of aseptic loosening is probably biological rather than mechanical and removing the subchondral bone plate may enhance biological fixation of cement to bone. With this in mind, perhaps it is time to revive removal of the subchondral bone as a standard part of acetabular preparation.

Aims: The purpose of this study is to review eight to eleven year results of a consecutive series of primary hybrid total hip arthroplasties. Methods: 193 patients (197 hips) received a cemented, polished, collarless, femoral component (Ra=1) in association with an uncemented acetabular component (166 grit-blasted titanium ßexible, 17 Ti mesh, and 14 Standard porous cups). Results: Mean age at surgery was 69.5 years (51–90). Most frequent diagnosis was osteoarthritis. Average follow-up was 9.2 years (8.5 to 11.2). 21 patients (23 hips) were lost to follow-up. 24 patients (25 hips) died prior to eight-years and none had undergone revision. Two femoral components, the only two with Grade D cement mantles, were revised for aseptic loosening at 23 and 111mos. One Ti mesh socket was revised for catastrophic poly wear at 113m and one ßexible cup for dislocation at 111m. The remaining 146 stems and cups were available for minimum 8year clinical review. 62 underwent complete radiographic review. Conclusions: Post-op Harris score was 89.3 (68–100). Pain relief was good or excellent in 95% of patients. X-rays revealed 1–2mm of femoral subsidence in 6 hips. Two cases of proximal (zones 7 and 1) femoral osteolysis were identiþed. A cemented polished femoral component and an uncemented acetabulum provide excellent clinical and radiographic results at average 9y follow up. Continued success appears likely in the absence of osteolysis or cement bone demarcation

We performed total hip replacement on 25 congenitally dislocated hips using corticocancellous bone from the femoral neck as a bone graft to reconstruct the deficient acetabulum. Two patients (two hips) died less than five years after surgery and three hips developed deep infection. We reviewed the remaining 20 hips at an average interval of 8.4 years (5.2 to 12.9). Their functional scores (modified Merle d'Aubigne and Postel) averaged 5.7 for pain, 5.1 for walking ability, and 4.2 for range of motion. Radiographs showed union and remodelling of the grafted bone in every case. There were radiological signs of aseptic loosening in three sockets (15%) without collapse of the grafted bone. None of the femoral stems was loose. No revision operation has been performed. Our results confirm the usefulness in the long term of femoral autografts for severe acetabular deficiency

Aims

The aim of this modified Delphi process was to create a structured Revision Hip Complexity Classification (RHCC) which can be used as a tool to help direct multidisciplinary team (MDT) discussions of complex cases in local or regional revision networks.

Methods

The RHCC was developed with the help of a steering group and an invitation through the British Hip Society (BHS) to members to apply, forming an expert panel of 35. We ran a mixed-method modified Delphi process (three rounds of questionnaires and one virtual meeting). Round 1 consisted of identifying the factors that govern the decision-making and complexities, with weighting given to factors considered most important by experts. Participants were asked to identify classification systems where relevant. Rounds 2 and 3 focused on grouping each factor into H1, H2, or H3, creating a hierarchy of complexity. This was followed by a virtual meeting in an attempt to achieve consensus on the factors which had not achieved consensus in preceding rounds.
