Background. Sequentially annealed, highly crosslinked polyethylene (HXLPE) has been used clinically in total knee arthroplasty (TKA) for over a decade[1]. However, little is known about the reasons for HXLPE revision, its surface damage mechanisms, or its in vivo oxidative stability relative to conventional polyethylene. We asked whether retrieved sequentially annealed HLXPE tibial inserts exhibited: (1) similar reasons for revision; (2) enhanced resistance to surface damage; and (3) enhanced oxidative stability, when compared with tibial inserts fabricated from conventional gamma inert sterilized polyethylene (control). Methods. Four hundred and fifty-six revised tibial inserts in two cohorts (sequentially annealed and conventional UHMWPE control) were collected in a multicenter retrieval program between 2000 and 2016. We controlled for implantation time between the two cohorts by excluding tibial inserts with a greater implantation time than the longest term sequentially annealed retrieval (9.5 years). The mean implantation time (± standard deviation) for the sequentially annealed components was 1.9 ± 1.7 years, and for the control inserts, 3.4 ± 2.7 years (Figure 1). Reasons for HXLPE revision were assessed based on medical records, radiographs, and examinations of the retrieved components. Surface damage mechanisms were assessed using the Hood method[2]. Oxidation was measured at the bearing surface, the backside surface, the anterior and posterior faces, as well as the post (when available) using FTIR (ASTM F2102). Surface damage and oxidation analyses were available for 338 of the components. We used nonparametric statistical testing to analyze for differences in oxidation and surface damage when adjusting for polyethylene formulation as a function of implantation time. Results. The tibial inserts in both cohorts were revised most frequently for loosening, infection, and instability. Instability was observed more frequently in inserts without a stabilizing post. In both cohorts, the most commonly observed surface damage mechanisms were burnishing, pitting, and scratching. Delamination was rare and only observed in 2 sequentially annealed inserts and 7 inserts in the control cohort. We observed six cases of posterior condyle fracture, which was always associated with instability (Figure 2). 5/6 of the fracture cases did not have a stabilizing post. Oxidation indices of the sequentially annealed inserts were, on average, low (ASTM oxidation index < 1) and not significantly different than the control inserts on the bearing surface and anterior/posterior face (Figure 3). Discussion. The findings of this study document the reasons for revision, surface damage mechanisms, and oxidative behavior of sequentially annealed HXLPE for TKA. We observed evidence of low in vivo oxidation in both retrieved sequentially annealed HXLPE and control tibial inserts. We found no association between the levels of oxidation and clinical performance of the HXLPE tibial components. However, because of the short-term follow-up, analysis of longer-term retrievals may be appropriate

Unicompartmental knee arthroplasty (UKA) is effective for patients with isolated compartment osteoarthritis, however the procedure has higher revision rates. Long-term survivorship and accurate characterisation of revision reasons are limited by a lack of long-term data and standardised revision definitions. We aimed to identify survivorship, risk factors and revision reasons in a large UKA cohort with up to 20 years follow-up.

Patient, implant and revision details were recorded through clinical and radiological review for 2,137 consecutive patients undergoing primary medial UKA across Auckland, Canterbury, Counties Manukau and Waitematā DHB between 2000 and 2017. Revision reasons were determined from review of clinical, laboratory, and radiological records for each patient using a standardised protocol. To ensure complete follow-up data was cross-referenced with the New Zealand Joint Registry to identify patients undergoing subsequent revision outside the hospitals. Implant survival, revision risk and revision reasons were analysed using Cox proportional-hazards and competing risk analyses.

Implant survivorship at 15 years was comparable for cemented fixed-bearing (cemFB; 91%) and uncemented mobile-bearing (uncemMB; 91%), but lower for cemented mobile-bearing (cemMB; 80%) implants. There was higher incidence of aseptic loosening with cemented implants (3–4% vs. 0.4% uncemented, p<0.01), osteoarthritis (OA) progression with cemMB implants (9% vs. 3% cemFB/uncemMB; p<0.05) and bearing dislocations with uncemMB implants (3% vs. 2% cemMB, p=0.02). Compared with the oldest patients (≥75 years), there was a nearly two-fold increase in risk for those aged 55–64 (hazard ratio 1.9; confidence interval 1.1-3.3, p=0.03). No association was found with gender, BMI or ASA.

Cemented mobile-bearing implants and younger age were linked to lower implant survivorship. These were associated with disease progression and bearing dislocations. The use of cemented fixed-bearing and uncemented mobile-bearing designs have superior comparable long-term survivorship.

Aims. Unicompartmental knee arthroplasty (UKA) has higher revision rates than total knee arthroplasty (TKA). As revision of UKA may be less technically demanding than revision TKA, UKA patients with poor functional outcomes may be more likely to be offered revision than TKA patients with similar outcomes. The aim of this study was to compare clinical thresholds for revisions between TKA and UKA using revision incidence and patient-reported outcomes, in a large, matched cohort at early, mid-, and late-term follow-up. Methods. Analyses were performed on propensity score-matched patient cohorts of TKAs and UKAs (2:1) registered in the New Zealand Joint Registry between 1 January 1999 and 31 December 2019 with an Oxford Knee Score (OKS) response at six months (n, TKA: 16,774; UKA: 8,387), five years (TKA: 6,718; UKA: 3,359), or ten years (TKA: 3,486; UKA: 1,743). Associations between OKS and revision within two years following the score were examined. Thresholds were compared using receiver operating characteristic analysis. Reasons for aseptic revision were compared using cumulative incidence with competing risk. Results. Fewer TKA patients with ‘poor’ outcomes (≤ 25) subsequently underwent revision compared with UKA at six months (5.1% vs 19.6%; p < 0.001), five years (4.3% vs 12.5%; p < 0.001), and ten years (6.4% vs 15.0%; p = 0.024). Compared with TKA, the relative risk for UKA was 2.5-times higher for ‘unknown’ reasons, bearing dislocations, and disease progression. Conclusion. Compared with TKA, more UKA patients with poor outcomes underwent revision from early to long-term follow-up, and were more likely to undergo revision for ‘unknown’ reasons, which suggest a lower clinical threshold for UKA. For UKA, revision risk was higher for bearing dislocations and disease progression. There is supporting evidence that the higher revision UKA rates are associated with lower clinical thresholds for revision and additional modes of failure. Introduction: Revision hip arthroplasty places a significant burden on hospital resources. Huge pressure is being placed on the orthopaedic community to alter practices with respect to implant selection and bearing surfaces in order to try to reduce the likelihood of revision due to aseptic socket loosening. To date there is little clinical evidence to support these changes.

Aim: To review the case mix requiring revision surgery at a specialist arthroplasty unit in order to identify the common reasons for failure of primary arthroplasties.

Methods: A retrospective single surgeon review was performed to identify patients who had undergone revision hip surgery over the study period. The reasons for revision were identified for all cases. Particular attention was paid to the cases with aseptic socket loosening to determine the time to revision for these cases.

Results: 176 revision procedures were performed between October 2001 and May 2005. In 74 (42%) cases aseptic socket loosening was identified (average socket survival 15.4 years). In 16 cases this was the sole cause for revision. In 58 cases the femoral component was also loose. 102 (58%) cases were performed for other reasons. Dislocation was the cause in 14%, femoral component loosening in 20%, infection in 18% and fracture in 6%. Aseptic loosening of cemented sockets less than 10 years old was only seen in 7 (4%) cases.

Conclusions: Aseptic loosening of cemented sockets less than 10 years old was the least common cause of revision in this series. Cemented polyethylene acetabular components continue to provide a satisfactory bearing surface on the acetabular side of total hip arthroplasties. We recommend caution when interpreting the information provided with new products with respect to the benefits of different fixation and bearing surfaces for the majority of patients.

Introduction

A key outcome measured by national joint registries are revision events. This informs best practice and identifies poor-performing surgical devices. Although registry data often record reasons for revision arthroplasty, interpretation is limited by lack of standardised definitions of revision reasons and objective assessment of radiologic and laboratory parameters. Our study aim was to compare reasons for unicompartmental knee arthroplasty (UKA) revision reported to the New Zealand Joint Registry (NZJR) with reasons identified by independent clinical review.

Methods

A total of 2,272 patients undergoing primary medial and lateral UKA at four large tertiary hospitals between 2000 and 2017 were included. A total of 158 patients underwent subsequent revision with mean follow-up of 8 years. A systematic review of clinical findings, radiographs and operative data was performed to identify revision cases and to determine the reasons for revision using a standardised protocol. These were compared to reasons reported to the NZJR using Chi-squared and Fisher exact tests.

Introduction and Aim: Following the establishment of the Australian Orthopaedic Association National Joint Replacement Registry, the Australian orthopaedic community has quality prosthesis specific information on knee replacement. This presentation details the demographics of primary total knee replacement, types of prostheses used, methods of fixation and the incidence of, and reasons for, early revision.

Results: Over 36,000 primary total knee replacements with almost 400 subsequent revisions were recorded. Primary total knee replacements were undertaken more commonly in females (56.4%), mostly for osteoarthritis (95.9%), with a mean subject age of 69.6 years.

The Registry recorded 56 different knee prostheses with the 10 most common accounting for 85.5% of all procedures. The patella was not replaced in the majority of cases (58.5%), however this varied considerably with prosthesis type and method of fixation. Cement fixation of the tibial component occurred in 76.9% of cases and the femoral component in 49.5%. Most commonly the insert was fixed (71.3%) and minimally stabilised (86.7%). Posterior stabilised inserts were used in 12.8% of primary cases. The cumulative revision rate at one year was 1.0% and 2.1% at two years. Early revision was minor in 54.1% of cases and major in the remainder. The most common reasons for minor revision were patello-femoral pain (27.1%) and infection (21.7%); for major revision, early loosening (40.2%) and infection (27.5%). Prosthesis type, patella use, method of fixation, degree of constraint and the use of fixed, rotating and/or sliding inserts did not significantly affect revision rates at this early stage.

Conclusion: Although variation is seen in early revision rates depending on the prosthesis type, patella use, method of fixation and other prosthesis specific characteristics, these differences are currently not significant.

Aims. The aim of this study was to assess the clinical and radiological results of patients who were revised using a custom-made triflange acetabular component (CTAC) for component loosening and pelvic discontinuity (PD) after previous total hip arthroplasty (THA). Methods. Data were extracted from a single centre prospective database of patients with PD who were treated with a CTAC. Patients were included if they had a follow-up of two years. The Hip Disability and Osteoarthritis Outcome Score (HOOS), modified Oxford Hip Score (mOHS), EurQol EuroQoL five-dimension three-level (EQ-5D-3L) utility, and Numeric Rating Scale (NRS), including visual analogue score (VAS) for pain, were gathered at baseline, and at one- and two-year follow-up. Reasons for revision, and radiological and clinical complications were registered. Trends over time are described and tested for significance and clinical relevance. Results. A total of 18 females with 22 CTACs who had a mean age of 73.5 years (SD 7.7) were included. A significant improvement was found in HOOS (p < 0.0001), mOHS (p < 0.0001), EQ-5D-3L utility (p = 0.003), EQ-5D-3L NRS (p = 0.013), VAS pain rest (p = 0.008), and VAS pain activity (p < 0.0001) between baseline and final follow-up. Minimal clinically important improvement in mOHS and the HOOS Physical Function Short Form (HOOS-PS) was observed in 16 patients (73%) and 14 patients (64%), respectively. Definite healing of the PD was observed in 19 hips (86%). Complications included six cases with broken screws (27%), four cases (18%) with bony fractures, and one case (4.5%) with sciatic nerve paresthesia. One patient with concurrent bilateral PD had revision surgery due to recurrent dislocations. No revision surgery was performed for screw failure or implant breakage. Conclusion. CTAC in patients with THA acetabular loosening and PD can result in stable constructs and significant improvement in functioning and health-related quality of life at two years' follow-up. Further follow-up is necessary to determine the mid- to long-term outcome. Cite this article: Bone Jt Open 2023;4(2):53–61

The 2021 Australian Orthopaedic Association National Joint Replacement Registry report indicated that total shoulder replacement using both mid head (TMH) length humeral components and reverse arthroplasty (RTSA) had a lower revision rate than stemmed humeral components in anatomical total shoulder arthroplasty (aTSA) - for all prosthesis types and diagnoses. The aim of this study was to assess the impact of component variables in the various primary total arthroplasty alternatives for osteoarthritis in the shoulder. Data from a large national arthroplasty registry were analysed for the period April 2004 to December 2020. The study population included all primary aTSA, RTSA, and TMH shoulder arthroplasty procedures undertaken for osteoarthritis (OA) using either cross-linked polyethylene (XLPE) or non-cross-linked polyethylene (non XLPE). Due to the previously documented and reported higher revision rate compared to other anatomical total shoulder replacement options, those using a cementless metal backed glenoid components were excluded. The rate of revision was determined by Kaplan-Meir estimates, with comparisons by Cox proportional hazard models. Reasons for revision were also assessed. For a primary diagnosis of OA, aTSA with a cemented XLPE glenoid component had the lowest revision rate with a 12-year cumulative revision rate of 4.7%, compared to aTSA with cemented non-XLPE glenoid component of 8.7%, and RTSA of 6.8%. The revision rate for TMH was lower than aTSA with cemented non-XLPE, but was similar to the other implants at the same length of follow-up. The reason for revision for cemented aTSR was most commonly component loosening, not rotator cuff deficiency. Long stem humeral components matched with XLPE in aTSA achieve a lower revision rate compared to shorter stems, long stems with conventional polyethylene, and RTSA when used to treat shoulder OA. In all these cohorts, loosening, not rotator cuff failure was the most common diagnosis for revision

The objective of this study was to assess the clinical and radiological results of patients who were revised using a new generation custom-made triflange acetabular component (CTAC) for component loosening and large acetabular defect (Paprosky 3A and 3B) after previous total hip arthroplasty (THA). New generation CTACs involve the use of patient-specific drill guides and incorporate three-dimensional printed bone models, enhancing precision during surgical implantation. Data were extracted from a single centre prospective database of patients with large acetabular defects who were treated with a new generation CTAC. Patients were included if they had a minimum follow-up of five years. The modified Oxford Hip Score (mOHS), EurQol EuroQoL five-dimension three-level (EQ-5D-3L) utility, and Numeric Rating Scale (NRS), including visual analogue score (VAS) for pain, were gathered at baseline, and at two- and five-year follow-up. Reasons for revision, and radiological and clinical complications were registered. Trends over time are described and tested for significance (p<0.05). A total of 49 (70%) of 70 patients with a mean age of 73.5 years (SD 7.7) had a complete follow-up of 5 years. A significant improvement was found in HOOS, mOHS, EQ-5D-3L utility and NRS, VAS pain rest and activity between baseline and final follow-up. Complications included 8 cases with loosening screws, 4 with bony fractures, 4 periprosthetic infections and 2 cases with dislocation. One patient with bilateral pelvic discontinuity had revision surgery due to recurrent dislocations. No revision surgery was performed for screw failure or implant breakage. New generation CTAC in patients with THA acetabular loosening and massive acetabular bone loss (Paprosky 3A and 3B) can result in stable constructs and significant improvement in functioning and health-related quality of life at five years’ follow-up

Outcomes following knee arthroplasty are typically defined as implant survivorship at defined timepoints, or revision incidence over time. These estimates are difficult to conceptualise, and lack context for younger patients with more remaining years of life. We therefore aimed to determine a ‘lifetime’ risk of revision as a more useful metric for total (TKA) and unicompartmental knee arthroplasty (UKA). The New Zealand Joint Registry was used to identify 96,497 primary TKAs and 13,481 primary UKAs performed between 1999 and 2019. Patient mortality and revision incidence were also extracted. Estimates of lifetime risk were calculated using an actuarial lifetable method. The estimates were stratified by age and gender. Reasons for revision were categorised using previously published standardised definitions. The lifetime risk of UKA revision was two-fold higher than TKA across all age groups (range 3.7-40.4% UKA, 1.6-22.4% TKA). Revision risk was higher for males with TKA (range 3.4%-25.2% males, 1.1%-20% females), but higher for females with UKA (range 4.3%-43.4% vs. 2.9%-37.4% for males). Revision due to infections were higher for TKA (1.5% males, 0.7% females) compared with UKA (0.4% males, 0.1% females). The increased risk in younger UKA patients was associated with higher incidence of aseptic loosening (UKA 2%, TKA 1%) and ‘unexplained pain’ (UKA 2%, TKA 0.2%). The risk for UKA was two-fold higher than TKA, and this was partially explained by a higher proportion of revisions due to ‘unexplained pain’. For TKA, males had higher risk of revision, in contrast to UKA where females had higher risk; this gender difference was associated with higher incidence of infections with TKA. Younger age, gender and higher ASA status were also associated with increased lifetime risk of UKA revision. Lifetime risk of revision can provide a meaningful measure of arthroplasty outcomes to aid patient counselling

This study aimed to quantify self-reported outcomes and walking gait biomechanics in patients following primary and revision THA. The specific goals of this study were to investigate: (i) if primary and revision THA patients have comparable preoperative outcomes; and (2) if revision THA patients have worse postoperative outcomes than primary THA patients. Forty-three patients undergoing primary THA for osteoarthritis and 23 patients undergoing revision THA were recruited and followed longitudinally for their first 12 postoperative months. Reasons for revision were loosening (73%), dislocation (9%), and infection (18%). Patients completed the Hip dysfunction and Osteoarthritis Outcome Score (HOOS), and underwent gait analysis preoperatively, and at 3 and 12 months postoperatively. A 10 camera motion analysis system (V5 Vantage, Vicon, UK) recorded marker trajectories (100 Hz) during walking at self- selected speeds. A generic lower-body musculoskeletal model (Gait2392) was scaled using principal component analysis [1] and the inverse kinematics tool in Opensim 3.3 was used to compute joint angles for the lower limbs in the sagittal plane. Independent samples t-test were used to compare patient reported outcomes between the primary and revision groups at each timepoint. Statistical parametric mapping was used to compare gait patterns between the two groups at each timepoint. Preoperatively, patients undergoing primary THA reported significantly worse pain (p<0.001), symptoms (p<0.001), function (p<0.001), and quality of life (p=0.004). No differences were observed at 3 and 12 months postoperatively between patients who had received a primary or revision THA. The only observed difference in gait pattern was that patients with a revision THA had reduced hip extension at 3 months, but no differences were observed preoperatively and 12 months. Despite the suggestions in the literature that revision THA is bound to have worse outcomes compared to primary THA, we found no differences in in patient-reported outcomes and gait patterns at 12 months postoperatively. This suggests that it may be possible, in some circumstances, for patients following revision THA to achieve similar outcomes to their peers undergoing primary THA

Aims. Adverse spinal motion or balance (spine mobility) and adverse pelvic mobility, in combination, are often referred to as adverse spinopelvic mobility (SPM). A stiff lumbar spine, large posterior standing pelvic tilt, and severe sagittal spinal deformity have been identified as risk factors for increased hip instability. Adverse SPM can create functional malposition of the acetabular components and hence is an instability risk. Adverse pelvic mobility is often, but not always, associated with abnormal spinal motion parameters. Dislocation rates for dual-mobility articulations (DMAs) have been reported to be between 0% and 1.1%. The aim of this study was to determine the early survivorship from the Australian Orthopaedic Association National Joint Replacement Registry (AOANJRR) of patients with adverse SPM who received a DMA. Methods. A multicentre study was performed using data from 227 patients undergoing primary total hip arthroplasty (THA), enrolled consecutively. All the patients who had one or more adverse spine or pelvic mobility parameter had a DMA inserted at the time of their surgery. The mean age was 76 years (22 to 93) and 63% were female (n = 145). At a mean of 14 months (5 to 31) postoperatively, the AOANJRR was analyzed for follow-up information. Reasons for revision and types of revision were identified. Results. The AOANJRR reported two revisions: one due to infection, and the second due to femoral component loosening. No revisions for dislocation were reported. One patient died with the prosthesis in situ. Kaplan-Meier survival rate was 99.1% (95% confidence interval 98.3 to 100) at 14 months (number at risk 104). Conclusion. In our cohort of patients undergoing primary THA with one or more factor associated with adverse SPM, DM bearings conferred stability at two years’ follow-up. In patients undergoing THA, prevalence of adverse SPM has been shown to be as high as 41%. Stiff lumbar spine, large posterior standing pelvic tilt and severe sagittal spinal deformity have been identified as risk factors for increased hip instability. Dislocation rates for dual mobility articulations have been reported to be 0% to 1.1%. The aim of this study was to determine the early survivorship from the Australian National Joint Replacement Registry (AOANJRR) of patients with adverse SPM who received a dual mobility articulation. A multicentre study was performed using data from 229 patients undergoing primary THA, enrolled consecutively. All the patients who had one or more adverse spine or pelvic mobility parameters had a dual mobility articulation inserted at the time of their surgery. Average age was 76 (22 to 93) years and 63% were female. At a mean of 2.1 (1 – 3.3) years post-op, the AOANJRR was analysed for follow-up. Reasons for revision and types of revision were identified. The AOANJRR reported two revisions. One due to infection and the second due to femoral component loosening. No revisions for dislocation were reported. One patient died with the prosthesis in situ. Kaplan Meier survival was 99.3% (CI 98.3% − 100%) at 2 years. DM bearings reduce the risk of dislocation of primary THA in patients with adverse spine and pelvic mobility

Introduction. Dual mobility (DM) total hip arthroplasty (THA) prostheses are designed to increase stability. In the setting of primary and revision THA, DM THA are used most frequently for dysplasia and instability diagnoses, respectively. As the use of DM THA continues to increase, with 8,031 cases logged in the American Joint Replacement Registry from 2012–2018, characterizing in vivo damage and clinical failure modes are important to report. Methods. Under IRB-approved implant retrieval protocol, 43 DM THA systems from 41 patients were included. Each DM THA component was macroscopically examined for standard damage modes. Clinically-relevant data, including patient demographics and surgical elements, were collected from medical records. Fretting and corrosion damage grading is planned, according to the Goldberg et al. classification system. Results. In this 43-retrieved implant series, there were 23 female and 17 male patients (n=1, unknown), with an average body mass index of 29 (range, 19–49), and average ages at index and revision of 63 years (range, 34–80) and 64 years (range, 38–88), respectively. The average duration of implantation was 12.9 months (range, 0.1–72.0). Reasons for revision included infection (n=11, 26%), mechanical complication (n=10, 23%), intraprosthetic dislocation (n=6, 14%), periprosthetic fracture (n=5, 12%), pain (n=4, 9%), acetabular-associated loosening (n=3, 7%), unknown (n=3, 7%), hematoma (n=2, 5%), leg length discrepancy (n=1, 2%), and inflammatory reaction (n=1, 2%); some cases included multiple reasons for revision. On articular surfaces, scratching was the most commonly observed damage mode on all components, with more than 40% of acetabular cup and femoral heads showing scratching damage (Figure 1A). Abrasion, burnishing, and pitting damage were also observed in more than 10% of acetabular cup and acetabular liner components; further, approximately 20% of polyethylene acetabular liners exhibited edge deformation damage. On backside surfaces, polyethylene acetabular liners showed the greatest damage, with more than 60% of components exhibiting abrasion, scratching, or pitting damage (Figure 1B). Conclusion. This series showed various reasons for revision as well as in vivo damage of retrieved DM systems following short-to-midterm implantation. Damage was observed on both articular and backside surfaces of the five components of DM THA. Modularity of DM THA prostheses may amplify rates of in vivo damage. Materials and methods: Based on data from the Norwegian Arthroplasty Register, we reviewed the results of uncemented femoral stems in Norway in the period 1987 to 1. April 2002. We compared these results to the results of the cemented Charnley monoblock stem, which still is the most commonly used femoral stem in Norway. Only prostheses used in more than one hundred hips were included. Based on these criteria, we identified a total of 7 856 primary total hip prostheses in fifteen different uncemented stems. With the Kaplan-Meier method, the survival of the primary prostheses were calculated and compared with each other and with the Charnley stem. The endpoint was revision in which the whole prostheses or the stem alone was removed or exchanged. The Cox regression analysis was used to adjust for differences in age, gender, diagnosis, former operations and profylactic antibiotics. Separate analyses for patients younger than 60 years and for stems with follow-up less than ten years were done. Reasons for revision in the uncemented Corail stem and the cemented Charnley stem was analysed. Results: The Corail stem, wich is the most commonly used uncemented stem in Norway (n=3590), had excellent long term results. 10 years survival in patients younger than 60 years was for the Corail 97,5% and for the Charnley 90,2% (p=0,001). Reasons for revision of the Corail were in most cases pain and repeated dislocations. Aseptic loosening was the dominant cause of failure of the Charnley stem. Several uncemented stems have results which are as good as or superior to the Charnley. The Femora, the Biofit, the Parhofer and the Harris Galante stems all had inferior results. Many uncemented stems with less than 10-years of follow-up have excellent 5 years survival. The 10 years survival of uncemented total hip arthroplasties, however was inferior to the all-cemented Charnley. Cup revisions due to aseptic loosening, and wear and/or osteolysis were the reasons for this. Discussion: The Corail stem has excellent long term results. Several newer stems have promising short- and medium term results. Due to inferior results of unce-mented cups, however, most Norwegian orthopaedic surgeons use cemented primary hip replacements

Introduction. Three-dimensional (3D) printing of porous titanium implants marks a revolution in orthopaedics, promising enhanced bony fixation whilst maintaining design equivalence with conventionally manufactured components. No retrieval study has investigated differences between implants manufactured using these two methods. Our study was the first to compare these two groups using novel non-destructive methods. Materials and methods. We investigated 16 retrieved acetabular cups divided into ‘3D printed’ (n = 6; Delta TT) and ‘conventional’ (n = 10; Pinnacle Porocoat). The groups were matched for age, time to revision, size and gender (Table 1). Reasons for revision included unexplained pain, aseptic loosening, infection and ARMD. Visual inspection was performed to evaluate tissue attachment. Micro-CT was used to assess clinically relevant morphometric features of the porous structure, such as porosity, depth of the porous layer, pore size and strut thickness. Scanning electron microscopy (SEM) was applied to evaluate the surface morphology. Results. Significant differences (p = 0.0002) were found for all morphometric parameters (Table 2). Microscopic analysis revealed uniform beads over the backside of conventional implants, due to the manufacturing technique (Figure 1a). Conversely, beads of random size were found on 3D printed implants, representing a by-product of the manufacturing process, where some starting powder particles are not completely fused together (Figure 1b). The two groups showed similar tissue attachment (3D printed 76.9 ± 27.1%; conventional 73.8 ± 12.2%; p = 0.2635). Conclusion. This was the first study to analyse retrieved 3D printed orthopaedic implants. Differences were found between these and conventional implants, but both literature and registry data do not suggest a short-mid-term clinical issue with 3D printed components. Similar tissue on growth suggested a comparable behaviour with bone in situ. The key difference is the presence of the particles on 3D printed implants, whose clinical significance needs to be investigated. For any figures or tables, please contact the authors directly

Introduction. Historically, US arthroplasty revision rates are based on CMS data that cannot verify initial surgery date in patients under 65 years or laterality of revision. We calculated US one-year revision rates for primary total hip replacement (THR) using a representative cohort. Reasons for revision were documented. Methods. A multi-center cohort from US surgeons in 28 states collected sociodemographic data; medical, emotional, musculoskeletal comorbidities; BMI; and patient-reported pain and function (SF36, HOOS) for elective THR patients. Cases in 2011–2013 were matched with CMS data to ascertain 1 year revision through 2014. Predictors of revision were identified. Chart reviews to verify reasons for revision were performed. Results. Overall, 1.6% of 2926 primary THR surgeries were revised within 12 months. Mean age was 72 years, 59% female. No significant difference in pre-operative age, sex, BMI, pain, function, or emotional health was detected between revision and non-revision patients. Severe low back pain was twice as prevalent among revisions (23% vs. 11%; p<0.04) as was prior stroke (8.5% vs. 3%). Primary reasons for revision were infection (30%), mechanical failure including dislocation (26%), fracture (19%), metal ions (15%) and other (10%). In patients under 65 years of age, the distribution of reasons for revision are identical. Conclusion. The incidence of revision THR in the US within 12 months of the index procedure is 1.6%. Infection and mechanical failure, including dislocation, are the most common causes of early revision, followed by peri-prosthetic fracture. Patients with severe pre-op lumbar spine pain are at higher risk for early revision after THR. Abbreviations: total hip replacement (THR)

Introduction/Aims. The Exeter Stem can be used with metal femoral head that are made of either cobalt chrome, or stainless steel. The aim of this study was to compare the rates of revision of these two metal femoral head types when used with this femoral component. Method. Australian Orthopaedic Association National Joint Replacement Registry (AOANJRR) Data from September 1999 until December 2015 for all primary THRs using an Exeter or an Exeter v40 stem with the diagnosis of osteoarthritis were analysed. Only bearing couples that used a metal head with polyethylene were included. The cumulative percent revision (CPR) calculated using Kaplan-Meier estimates were compared for the two metal head types. CPR were further analysed by age, polyethylene type and head size. Reasons for revision and types of revision were assessed. Results. There were 51666 THR that used Exeter or Exeter v40 stems of which 12554 had femoral heads made of cobalt chrome and 39112 had heads made of stainless steel. There was no difference in the rate of revision overall when head types were compared. There was also no difference in CPR between the two head types with age. Hips that used cobalt chrome heads had a higher CPR than stainless steel heads when these were used against non-crosslinked polyethylene. When heads with a diameter of 32mm or greater were compared, those made of cobalt chrome had a higher rate of revision than stainless steel HR 1.38 (1.15, 1.66) P<0.001). Conclusion. There was no difference comparing cobalt chrome or stainless-steel heads, except where non-crosslinked polyethylene was used, or where the heads size was 32mm or greater. In these comparisons, heads made from cobalt chrome had a higher rate of revision

Introduction. Modular-neck hips have twice the rate of revision compared to fixed stems. Metal related pathology is the second most common reason for revision of implants featuring titanium stems with cobalt chrome necks. We aimed to understand the in-vivo performance of current designs and explore the rationale for their continued use. Methods. This study involved the examination of 200 retrieved modular-neck hips grouped according to the material used for their neck and stem. Groups A, B and C had neck/stems featuring CoCr/beta Ti-alloy (TMZF), CoCr/Ti6Al4V-alloy, and Ti6Al4V/Ti6Al4V respectively. Reasons for revision included pain, elevated metal ion levels and fluid collection. The stem-neck interface was assessed for severity of fretting/corrosion using metrology methods to compute linear wear penetration rate. Results. All retrieved implants with a CoCr/Ti combination showed evidence of moderate-severe fretting corrosion at the neck-stem junction. Maximum penetration depth rate in group A (median of 17.9μm/year) was higher than that in group B (median=5.8μm/year); p=0.0012 and group C (median=1.55μm/year), p=0.0095. Implant failure occurred 2-fold earlier in modular-neck hips with TMZF-alloy stems than Ti6Al4V. There was a strong correlation between severity of damage and time to revision in the TMZF group A (p < 0.0001) and between taper damage and Co levels (p < 0.0001) and Cr levels (p < 0.0012). Patient and implant data did not correlate with the amount of material loss observed, (p>0.05). Discussion. The findings are in agreement with registry data as designs with CoCr/Ti at neck/stem interfaces had the highest levels of fretting and corrosion and lower threshold for revision. Conclusion. Corrosion of metal orthopaedic implants remains of clinical concern to patients, surgeons and industry. In light of the findings, the continued use of modular-neck with a metal mismatch at the neck/stem junction is unfavourable and should be avoided

Introduction. The causes of revision total knee replacement are varied. There is a subset of these revision cases (poly wear and some cases of instability for example) where the ability to retain the metal femoral and tibial components and replacing just the polyethylene is an appealing option. We report on a series of RTKR where only the poly insert was replaced and the patients were followed for a minimum of ten years. Materials and Methods. Our study group consisted of 64 consecutive non infected RTKR patients who underwent a revision of the polyethylene alone between 1998 and 2006. All patients had been treated originally with the same cemented, CR, patella resurfaced primary TKR. Reasons for revision were 51 poly wear and/or osteolysis, 7 instability, and 6 other. The average time from the primary to the revision surgery was 9.1 years (range 2.2 to 16.1 years). All patients had an isolated poly liner change. No femurs or tibial trays were revised. Average age of the cohort at revision was 72.2 (range 48 to 88). Average BMI was 31.9 (range 23.6 to 43.9). There were 36 female patients. Of the group, 42 were ASA 1 or 2 and 22 were ASA 3. Pre-op alignment averaged 6.0 (range 2 varus to 8 valgus). Patients were followed for a minimum of 10 years (range 10 to 19 years). Results. At final follow up 13 patients had died, none were lost to follow up leaving 51 patients available for review. There were 7 reoperations in 6 patients (7 knees). Reasons for reoperation were: 4 aseptic tibial loosening at an average of 4.1 years (range 1.2 to 8.3) following first poly revision, two for polywear at an average 9 years post first poly revision, and one for deep infection (5.6 years post first poly revision). Knee society scores at 6 weeks post revision was 93.5 (range 38 to 100) and at final follow up for the non-revised knees was 91.6 (range 36 to 100). Conclusion. In appropriate settings, where the femoral and tibial components are satisfactorily aligned and well fixed and the soft tissues can be balanced, a poly change alone can provide a durable solution for the revision total knee patient. The ability to retain the metal components has advantages in terms of patient morbidity and cost

Introduction. The revision rate of unicompartmental knee replacement (UKR) in national joint registries is much higher than that of total knee replacements and that of UKR in cohort studies from multiple high-volume centres. The reasons for this are unclear but may be due to incorrect patient selection, inadequate surgical technique, and inappropriate indications for revision. Meniscal bearing UKR has well defined evidence based indications based on preoperative radiographs, the surgical technique can be assessed from post-operative radiographs and the reason for revision from pre-revision radiographs. However, for an accurate assessment aligned radiographs are required. The aim of the study was to determine why the revision rate of UKR in registries is so high by undertaking a radiographic review of revised UKR identified by the United Kingdom's (UK) National Joint Registry (NJR). Methods. A novel cross-sectional study was designed. Revised medial meniscal bearing UKR with primary operation registered with the NJR between 2006 and 2010 were identified. Participating centres from all over the country provided blinded pre-operative, post-operative, and pre-revision radiographs. Two observers reviewed the radiographs. Results. Radiographs were provided for 107 revised UKR from multiple centres. The recommended indications were not satisfied in 30%. The most common reason was the absence of bone-on-bone arthritis, and in 16 (19%) the medial joint space was normal or nearly normal. Post-operative films were mal-aligned in 50%. Significant surgical errors were seen in 50%, with most errors attributable to tibial component placement and orientation. No definite reason for revision was identified in 67%. Reasons for revision included disease progression (10%), tibial component loosening (7%), dislocation of the bearing (7%), infection (6%) femoral component loosening (3%), and peri-prosthetic fracture (2% - one femur, one tibia). Discussion and Conclusion. This study found that improper patient selection, inadequate surgical technique, inappropriate revisions and poorly taken radiographs all contributed to the high revision rate. There is a misconception that UKR should be used for early OA. Bone-on-bone arthritis is a requirement and was definitely not present in about 20%. There were many surgical errors, particularly related to the tibial cut: The new instrumentation should reduce this. There was a high prevalence of mal-aligned radiographs. Revisions should be avoided unless there is a definite problem, as the outcome of revision is usually poor in this situation. 80% of UKR revisions could potentially be avoided if surgeons adhered to the recommended indications for primary and revision surgery, and used the recommended surgical techniques. This study therefore suggests that if UKR was used appropriately the revision rate would be substantially lower and probably similar to that of TKR

Aim. Aim of the study was to find out whether patients with positive minor criteria but without meeting the MSIS definition have a difference in the outcome after revision-surgery compared to patients without any MSIS-criteria? And does the reason for revision-surgery (eg. loosening) have an additional influence on the outcome parameters in patients with positive minor criteria?. Method. A retrospective matched-pair analysis with 98 patients who had undergone revision-surgery after TJA was performed. Forty-nine patients who showed 1 to 3 positive minor criteria (PMC) whereas 49 patients without any positive minor criteria (aseptic complications control group) were compared regarding re-revision-rate and revision-free survival. Patients were matched regarding sex, age, joint and comorbidities using the classification system by McPherson et al. Reasons for revisions were categorized in loosening, body wear, periprosthetic fracture/mechanical failure and soft-tissue complication. Endpoints were defined as re-revision for any cases. Results. In the group of patients with PMC 30.6% (n=15) had to undergo re-revision compared to 6.12% (n=3) in the true aseptic complication (AC) control group. The overall-survival in the PMC-group was 69.4% (95% CI: 47–69 months) and in the AC-group 93.9% (95% CI: 82–94 months) (p=0.001). In patients with PMC but not exchange of the implant the overall-survival was 94.1% (95%CI survival time: 71–88 months) and in comparison the cohort with exchange of the prosthesis showed an overall-survival of 56.25% (95% CI survival time: 29–53 months) (p=0.008). Conclusions. Our findings suggest that in dependence of the occurrence of prosthetic loosening even single positive minor-criteria may have an impact on the outcome after THA- and TKA revision-surgeries

Introduction. A modern total knee replacement system was introduced in 2012 in order to improve the mechanics of total knee replacement (TKR). The purpose of this study was to assess the early clinical and radiographic outcomes of patients who have been treated with this modern TKR system at two years postoperatively. Methods. Patients who received this TKR system between June of 2012 and September of 2014 were included in this prospective registry-based follow-up study. Demographics, component, and surgical data were collected. Radiographic outcomes were also assessed. Patient reported outcome measures (PROMs) of all patients with 2-year follow-up data were collected. Reasons for revision and overall survival at 2-years were evaluated. Results. The study population consisted of 290 patients (307 knees). The mean age was 66 years and 61% were women. 250 patients received cruciate retaining and 57 patients posterior stabilized implants. The median tibiofemoral axis was in 4° valgus and the femoral component was in 5° valgus and 2° flexion. The median coronal and sagittal alignment of the tibial component was −1° varus and −5° slope, respectively. Most patients had tibial components that fit the tibial resection. At one year, 7 of 80 knees had a radiolucency in the tibia or femur. At two years, two of 30 knees had a radiolucency in the tibia or femur. Mean PROMs for thirty-six patients at two years were: EQ-5D WHI 0.76 (range 0.31–1.00), EQ-5D VAS 82 (range 48–98), UCLA activity score 5.3 (range 2–10), KOOS pain 70.9 (22–100), symptoms 69 (14–96), ADL 74.2 (18–100), sports 51.4 (0–100), and QoL 59.8 (0–90). Cumulative 2-year implant survival was 97.1%. There were 7 revisions for infection and 2 revisions due to an unsatisfactory clinical outcome. Conclusion. Clinical and radiographic outcomes at two years were good, and overall implant survival was excellent

Introduction. We report on the outcome of a cemented flanged acetabular component at a minimum of 10 years post-operatively. Patients/Materials & Methods. Two hundred and three hips were reviewed in 194 consecutive patients who underwent primary total hip arthroplasty using this implant. Cases with acetabular defects requiring bone grafting were excluded. Functional and radiological data were prospectively recorded. Results. Eight hips were revised, 66 hips were implanted in patients who died prior to their 10-year review and 129 hips remain in-situ at a minimum of 10 years. Reasons for revision were periprosthetic fracture due to trauma in 4 cases, recurrent dislocation in 2 cases, infection in 1 case and on-going pain in 1 case (in which infection was suspected but excluded). No hips were revised for aseptic loosening. Oxford and Harris hip scores demonstrated significant clinical improvement. Sixty four percent of the acetabular components were free of lucent lines. In the remainder, the vast majority exhibited lucencies in zone 1 only. Kaplan-Meier survivorship for aseptic loosening was 100% at 12.5 years and for all causes was 97.8% at 12.5 years (95% CI 95.6% – 100%) with 40 cases remaining at risk. Discussion. Data from our study adds further weight to the evidence from joint registries as to the merits of cemented acetabular components in primary THR. Conclusion. The clinical, radiological and survivorship outcomes of this component are excellent

Introduction:. We present the early results of 220 Rebalance Total Ankle Replacements performed in 6 centres in 4 different countries. Methods:. The Rebalance Total Ankle Replacement is a new 3 component uncemented mobile bearing prosthesis with a surface coating of ‘bonemaster’ and an ‘e’ poly bearing. The prosthesis was released in a limited way in May 2011. Since then 220 replacements have been implanted in 218 patients in 6 centres in 4 different countries (UK, Sweden, Canada, Czech Republic). All the x-rays and case notes were reviewed.63 prostheses have a minimum follow up of 2 years. Outcome measures included revision of the prosthesis, and the incidence of progressive and non-progesssive radiolucent lines around the prosthesis. Results:. 7 patients underwent revision or are awaiting revision of their prosthesis. Reasons for revision included loosening(2), infection(1), malposition(2), periprosthetic fracture(1), impingement(1). 9 patients had non-progressive radiolucent lines and 2 patients had progressive radiolucent lines. 1 patient with progressive radiolucent lines is waiting revision for suspected infection. 8 patients suffered fractures to the lateral malleolus and 4 to the medial malleolus. There were no cases of balloon osteolysis. 29 patients had pre-op AOFAS scores= 41(12–67). 21 had 1 year post-op AOFAS scores= 73.5(26–100). 12 had 2 year AOFAS scores= 75.5(50–100). Conclusion:. The early results are encouraging and we believe support its wider use

Introduction. Cemented stems have given good results with no difference in outcome when comparing taper slip and composite beam designs in short-term randomised trials. We aimed to ascertain differences in outcome between the different cemented design philosophies using a large-scale registry analysis. Materials and Methods. A retrospective cohort study of National Joint Registry of England and Wales (NJREW) was conducted. The study population included all primary total hip operations performed in the UK from 1 April 2003 to 31 September 2012. All cemented stems were identified and categorised as taper slip (polished) or composite beam (non-polished). Results. Non-polished stems accounted for 16.6% of all primary cemented hip replacements with polished stems making up the remainder. Mean age for non-polished stems was 73.0 years and 71.8 for the polished group (p<0.001) suggesting selection bias. Introduction:. This study evaluates the mid-term results of first metatarsophalangeal joint replacement (MTPJR) for hallux rigidus using the Toefit-Plus™ prosthesis. Methods:. We prospectively studied the outcomes of 86 MTPJR in 73 patients using the AOFAS-HMI score and radiological follow up over a period from 2006 to 2013, with surgeries performed by a single surgeon at two centres. Patients were reviewed, scored and radiographs obtained pre-operatively and then at intervals of 6 weeks, 6 months, 12 months and then yearly. The mean follow up was 33 months (2–72). Results:. The mean AOFAS score of the patients not requiring revision at 1 year was 92, at 2 years was 94, at 3 years was 91, at 4 years was 99, at 5 years was 93, at 6 years was 100 and at 7 years was 97. 18 joints have either been revised or listed for revision giving a revision rate of 21%; this occurred at a mean of 33 months post-surgery. Reasons for revision included loosening of components in 13, infection in 1, dislocation in 2, malalignment in 1 and persistent pain in 1. Eight patients sustained intra-operative fractures requiring circlage wiring, of which 7 went on to union and one required revision. 25 patients had evidence of radiological loosening of which 22 were around the phalangeal component and 3 were around the metatarsal component. Conclusions:. First MTPJ replacement resulted in improved outcomes in patients with hallux rigidus who do not require revision in the medium term, however the revision rate is unacceptably high and as such we have discontinued use of this prosthesis. Radiological loosening of the components is high and needs monitoring for progression which may necessitate revision

The inmate population is a unique cohort with several healthcare-related challenges. International studies have demonstrated higher rates of infectious diseases, chronic diseases and psychiatric disorders in inmates when compared to general population. However, little is known about the outcomes following total joint arthroplasty in this population. This retrospective chart review aims to outline the differences in clinical outcomes after hip and knee total joint arthroplasty in the Kingston inmate population compared to the national population standard. A list of all inmate inpatient hospital visits with diagnostic/procedure codes pertaining to total joint arthroplasty within the last ten years was obtained through a computer-based search of the Kingston General Hospital Discharge Abstract Database(DAD). The patient charts were reviewed and demographic and outcome data pertinent to our study was collected. Data was compiled using Excel and imported into IBM SPSS for descriptive analysis. Twenty male inmate patients underwent 24 primary Total Hip Arthroplasties(THA) or Total Knee Arthroplasties(TKA) and one medial unicompartmental knee arthroplasty from May 2003 to January 2013. The average age was 58 with mean Body Mass Index(BMI) of 34. Median American Society of Anesthesiologist(ASA) score was 3 and mean Charlston Comorbidity Index was 3.92. The rates of HCV and HIV were 35%(n=5) and 0%, respectively. Average length of stay from time of initial procedure was 4.2 days. The overall revision rate was 24% (n=6). Reasons for revision included deep prosthetic infection (50%, n=3), aseptic loosening (17%, n=1), arthrofibrosis (17%, n=1) and late periprosthetic fracture (17%, n=1). Infection rates were reported at 16% (n=4); 75% of which were deep prosthetic infections requiring revision surgery. Other complications included ST-elevation myocardial infarction(STEMI) (n=1), and postoperative knee stiffness requiring manipulation under anesthesia(MUA) (n=1). Compared to Correctional Services Canada(CSC) data on male inmate health in 2012, our study population demonstrated a higher rate of HCV (35% vs. 9.4%), diabetes (30% vs. 4.2%) and overall cardiovascular and respiratory comorbidities. This may reflect the higher rate of comorbidities associated with osteoarthritis, such as BMI>25, which was evident in 95% of our study population vs. 64.5% in Canadian inmates. Total joint revision rates in our inmate study population was 24%, which is higher than the 2014 Canadian Joint Replacement Registry's yearly revision rate of 8.7% in THA and 5.2% in TKA. Our study population also demonstrated infection as the leading cause for revision at 50%, compared to 14.5% for THA and 19.6% for TKA in the general Canadian population. Further study of the complex biopsychosocial risk factors in the inmate population is warranted to better define pre-surgical risk assessment criteria

Purpose. Traditional total knee arthoplasty techniques have involved implantation of diaphyseal stems to aid in fixation expecially when using constrained polyethylene inserts. While the debate over cemented vs uncemented stems continues, the actual use of stems is considered routine. The authors' experience with cemented stemmed knee revisions in older patients with osteoporotic bone has been favorable. Our younger patients with press-fit stems from varying manufacturers have been plagued with a relatively high incidence of component loosening and stem tip pain in the tibia and occasionally thigh. We report the early results of the first 20 total knee revisions using press-fit metaphyseal filling sleeved stemless implants with constrained bearings. Methods. Twenty three patients with failed primary or revision total knees were assigned to receive stemless sleeved revision knee designs using the DePuy MBT/TC3 system. Reasons for revision included loosening, implant fracture, stiffness, instability, and stem pain. Twenty patients (ages ranging from 42–73) were successfully reconstructed without stems. Six knees with significant uncontained cavitary defects were included. Three patients with unexpectedly osteoporotic metaphyseal bone were revised with cemented stemmed implants and excluded. All cases used cement for initial fixation on the cut bone surface and fully constrained mobile bearing inserts. Results. Follow up ranged from six months to three years. All patients had radiographic evidence of well fixed stable implants on most recent examination. All four cases of revision for “end of stem pain” had complete resolution of symptoms within two weeks of revision surgery. Long leg anterior posterior mechanical alignment x-rays measured within two degrees of neutral in all cases. Knee Society Scores improved an average of 34 points. Clinical results for revision for stiffness had the lowest final scores post operatively. Conclusion. Stemmed total knee arthroplasty revision implants with or without cement are considered the standard for most revision reconstructions. Recently, primary total hip replacements using newer short metaphyseal stems have shown promising early clinical results. This case series of twenty total knee revisions using stemless press-fit metaphyseal sleeves shows similarly favorable outcomes. The complications of stemmed implants such as stem tip pain and difficulty of cemented stem removal can be avoided successfully in non-osteporotic bone reconstructions. With stable bony ingrowth visible on early post-operatyive radiographs, long term stable fixation even with constrained bearings is expected. Longer follow up will be needed to validate this technique for routine use

Aims

The Birmingham Hip Resurfacing (BHR) arthroplasty has been used as a surgical treatment of coxarthrosis since 1997. We present 20-year results of 234 consecutive BHRs performed in our unit.

Methods

Between 1999 and 2001, there were 217 patients: 142 males (65.4%), mean age 52 years (18 to 68) who had 234 implants (17 bilateral). They had patient-reported outcome measures collected, imaging (radiograph and ultrasound), and serum metal ion assessment. Survivorship analysis was performed using Kaplan-Meier estimates. Revision for any cause was considered as an endpoint for the analysis.

Introduction. Alternative bearings – metal-on-metal and ceramic-on-ceramic bearings have been introduced in the last decade with the aim to diminish wear and, subsequently, aseptic loosening and osteolysis. These bearings were aimed for younger, more active patients. Clinical results which would compare the performance of various alternative and traditional bearings are scarce. Methods. Between January 2000 and December 2002, we performed 487 total hip replacements in 474 patients using three types of bearings: metal-on-metal (MOM), metal-on-polyethylene (MOP), and ceramic-on-ceramic (COC). All patients received the same type of acetabular cup (Bicon-Plus, Plus Orthopedics) and same type of femoral component (SL-Plus, Plus Orthopedics). We used the hospital computer database for the evaluation of patient data and data on revision operations. Results. At a mean follow-up of 8.5 years (range, 6.8 to 9.9 years), eighteen hips were revised. Seventeen revisions were aseptic, and one was septic. The percentage of revision in the whole group was 3.7%. The percentage of revisions for individual groups of bearings varied and ranged from 1.5% in the MOP group, 4.1% in the COC group to 8.7% in the MOM group. Reasons for revisions were analyzed in details. We noticed that the percentage of aseptic loosening differed among the groups, with the MOM group having the highest percentage of revisions due to aseptic loosening. Conclusions. The results obtained for this large series of THRs with the same type of acetabular and femoral components, and differing only in the type of bearing, indicate that the combination of material of acetabular cup and femoral head affect the performance of the hip prosthesis

A revision of a first generation KMFTR prosthesis due to stem breakage is a problem oncologic surgeons are regularly faced with. We designed an adapter which allows us to connect new MUTARS components to the original KMFTR devices. Thus it is possible to bypass an exchange of the whole prosthesis. We used this adapter in 10 patients. Time of revision was in average 16.6 years after primary implantation of the KMFTR prostheses. Reasons for revision were femoral stem breakage (n = 5), breakage of the tibial component (n = 3) and periprosthetic fracture (n = 2, one femoral, one tibial). The femoral stem (3 cases) and the tibial stem (2 cases) as well as the tibial plateau and body (2 cases) could be replaced by MUTARS parts and conjoined with the remaining KMFTR devices. Three cases were converted to MUTARS total femur. Postoperative complications were one aseptic loosening and one cone-dislocation. Pre-incidence function was restored in all cases. The average Musculoskeletal-Tumour-Society-Score was 82.9% of normal function. The results show that the new adapter facilitates to restore pre-incidental extremity function by performing a relatively modest revision

Introduction. Total hip arthroplasties in younger patients often requires revision because these patients frequently have acetabular deficiencies, which hamper proper implantation of the cup essential for good long-term prosthesis survival. For 30 years, we have used a biological acetabular-reconstruction technique with bone-impaction grafting in all patients <50 years with an acetabular deficiency at surgery, always in combination with a cemented total hip implant. Methods. We evaluated all 150 consecutive patients (177 hips) < 50 years with an acetabular reconstruction by bone-impaction grafting surgically-treated from 1978–2004 at our clinic. Mean follow-up was 10.3 (range, 2.0–28.3) years with no patient lost to follow-up. Mean index surgery age was 38.1 (range, 16–49) years. Clinical, radiological, and statistical analysis of all patients was performed. Results. Twenty-eight of 177 hips were revised at a mean of 10.5 years (range, 5 days to 23.2 years). Reasons for revision were: aseptic loosening (n=17), septic loosening (n=3), recurrent dislocations (n=3), traumatic loosening (n=2), neuropathy (n=1), wear (n=1), and fracture (n=1). Ten-year and 15-year survival with endpoint revision of any component for any reason was 91% and 78%. Ten-year survival with endpoint aseptic loosening was 96% for the cup and 97% for the stem. Conclusion. Performing a total hip implant in combination with acetabular bone impaction grafting in younger patients with acetabular bone stock loss seems to be an attractive approach as the long-term results are acceptable and fulfill the NICE-criteria, showing a ten -year survival of more than 90% with endpoint revisions for any reason

Current evidence suggests that we should be moving away from Thompson's hemiarthroplasties for patients with intracapsular hip fractures. Furthermore, the use of cement when inserting these prostheses is controversial. This study aims to show the Inverness experience. We performed a retrospective review of all NHS Highland patients who underwent a hemiarthroplasty for an intracapsular neck of femur fracture over the last 15 years. Demographics and the use of cement were documented. Further analysis of this group was performed to identify any of these patients who required revision of their prosthesis. Patients requiring revision had their case-notes reviewed to identify the cause for further surgery. From 1996 until present 2221 patients from the Highland area had a hemiarthroplasty for an intracapsular neck of femur fracture. 1708 where female (77%) and 513 male (23%). The ages ranged from 28 years to 104 years (mean 80 years, median 81). 2180 of this group had their operations in Raigmore Hospital with the remaining 41 at various centres throughout Scotland. 623 (28%)had a cemented hemiarthroplasty, with the remaining 1578 (72%) having an uncemented Thompson's hemiarthroplasty. The revision rate for the cemented group was 2% (13 of 623 patients). In the uncemented group it was 0.4% (6 of 1578). Reasons from revision included dislocation, periprosthetic fracture, infection and pain. Current evidence from some joint registers regarding the use of Thompson's hemiarthroplasty in the elderly is discouraging. The use of bone cement in this group with multiple co-morbidities is not without it's risks. Our data suggests that uncemented Thompson's hemiarthroplasties in low demand elderly patients with multiple co-morbidities can yield excellent results with less risk to the patients

Aims

The primary objective of this registry-based study was to compare patient-reported outcomes of cementless and cemented medial unicompartmental knee arthroplasty (UKA) during the first postoperative year. The secondary objective was to assess one- and three-year implant survival of both fixation techniques.

Methods

We analyzed 10,862 cementless and 7,917 cemented UKA cases enrolled in the Dutch Arthroplasty Registry, operated between 2017 and 2021. Pre- to postoperative change in outcomes at six and 12 months’ follow-up were compared using mixed model analyses. Kaplan-Meier and Cox regression models were applied to quantify differences in implant survival. Adjustments were made for patient-specific variables and annual hospital volume.

Purpose: Resurfacing arthroplasty (RA) is becoming popular as an alternative to total hip arthroplasty (THA). Direct to consumer advertising reports good to excellent outcomes and patients sometimes seek RA on the assumption that it provides increased survivorship over THA. We report the 5 year survivorship of 25 RA procedures done at one institution. Method: 25 patients were prospectively followed after receiving a Conserve Plus RA through a direct lateral approach between 2002 and 2005. There were 22 males and 3 females with an mean age at surgery of 42 years. The average length of follow-up was 40 months. Failure was defined as revision of the components or a pending revision. Results: At a mean of 2 years follow-up, 3 patients had been revised and 2 more were scheduled for revision surgery for a failure rate of 20%. Reasons for revision included 1 neck fracture and 2 aseptically loose acetabular components. The 2 patients waiting for revision also had aseptically loose acetabular components. Patients receiving or requiring revision were operated on throughout the series and were not necessarily part of the surgical learning curve. Conclusion: RA of the hip done through a direct lateral approach in this series had an unacceptably high failure rate for aseptic loosening of the acetabular component

Background. Reasons for revision of metal-on-metal hip resurfacing arthroplasty (MoMHRA) have evolved with improving surgical experience and techniques. Early revisions were often due to fracture of the femoral neck while later revisions are associated with loosening and/or adverse local tissue reactions (ALTR) to wear debris. In some studies, revisions of MoMHRA with ALTR have been complicated by an increased risk of rerevision and poor outcome. The purpose of this study was to investigate the causes of failure and to identify factors that improve outcome following revision of a failed HRA. Methods. From 2001 to May 2015, 180 consecutive HRA revisions were performed in 172 patients. Ninety-nine primary surgeries were done at a HRA specialist centre (99/4211, revision rate: 2.4%), 81 elsewhere. Eight different HRA designs were revised mainly in females (60%). Components’ orientation was measured from radiographs using EBRA. Ion levels were used as a diagnostic tool since 2006 (n=153). Harris-Hip-Score (HHS) was obtained prerevision and at latest follow-up. The initial experience of the first 42 cases (Initial Group) was compared to cases 43–180 (Later Group). Patients of the Later group were noted to have less soft tissue damage, had significantly bigger THA heads implanted at surgery, were educated of the increased complication risk and some wore an abduction brace for 6 weeks. Results. All patients presented with some pain/discomfort. Mean time to revision was 38 months (0–160). Eight HRAs were revised for fracture and 8 for infection. The most common reason for revision was component malpositioning (acetabular 48%, excessive abduction and/or anteversion; femoral 10%) usually associated with high metal ions (62%). The most common intra-operative finding was ALTR (48%) followed by metallosis (36%) and impingement (29%). Metal sensitivity was suspected in 8 patients (6F/2M). There were gender-specific differences in component sizes and causes of failure, with a higher incidence of component malpositioning, osteolysis and elevated metal ions in women. Time to revision in patients with high metal ion levels was shorter with the ASR (21 months, SD:10) in comparison to the BHR (38 months, SD: 25) (p=0.05). For the whole cohort, HHS significantly improved post revision (93, 42–100) (p<0.001). Fourteen complications (9 dislocations; 5 infections) and 9 re-revisions occurred. Outcome {HHSpost-op (p=0.04), complication and re-revision rates (p=0.005)} was significantly better in the Later Group compared to the Initial group. The incidence of complications/re-revisions significantly reduced since the introduction of metal ions (p=0.004). The presence of ALTR did not significantly affect outcome (p=0.65). However, patients with ALTR in the Later group (n=51) had significant reduced complication(p=0.005) and re-revision(p=0.016) rates in comparison to those in the Initial Group. Conclusion. Component malpositioning is the most common cause of HRA failure. Metal ion measurements are an excellent tool to detect wear at an early stage. The revision analysis highlights the importance of surgical experience, indications and prosthesis design. Use of ion levels, big THA-heads and patient education/compliance were identified as factors improving outcome following HRA revision. Patients with soft tissue reactions can have good outcome if operated prior to extensive soft tissue destruction

The purpose of this study was to analyse the reason for failure of shoulder arthroplasties performed for osteoarthritis, looking specifically at factors associated with either hemi or total shoulder replacement. Over an eight-year period over 80 consecutive patients undergoing a Global hemi-arthroplasty or total shoulder arthroplasty were looked at. All patients underwent at least two years of follow up. All patients were assessed pre and post operatively using the Constant and ASES scoring systems. A bio-engineer or physiotherapist performed these evaluations. X-rays were analysed for demarcation, loosening and glenoid erosion. Failure was defined as the need for revision surgery. Reasons for revision were predominately pain, loosening and instability. In these instances the differences between hemi and total joint arthroplasty were recorded. A distinct pattern was present with several early revisions being performed for continuing pain in the patient group who underwent hemi-arthroplasty. In conclusion, this study supports total shoulder replacement as the operation of choice, in suitable patients, as compared to hemi-arthroplasty. This would appear to reduce the risk of the need for early revision on account of pain. This must be weighed against the potential for late glenoid loosening

Background. Unicompartmental knee arthroplasty provides a good alternative to total knee arthroplasty in patients with isolated medial compartment osteoarthritis. Reported ten-year survival for the Oxford medial unicompartmental knee arthroplasty is variable, ranging from 80.2% to 97.7% in the originator series. The aim of this study was to determine the survival and reasons for revision of the Oxford medial unicompartmental knee arthroplasty when performed at a specialist orthopaedic centre. Methods. Details of consecutive patients undergoing Oxford unicompartmental knee arthroplasty at our centre between January 2000 and December 2009 were collected prospectively. Failure of the implant was defined as conversion to total knee arthroplasty. Survival was determined using the Kaplan-Meier method. Results. There were 494 Oxford unicompartmental knee arthroplasties implanted in 425 patients for isolated medial compartment osteoarthritis. Mean age was 62.8 yrs (range 34.6-90.1 yrs) and 53.4% were female. During a mean follow-up time of 3.0 yrs (range 0.5-9.2 yrs), twenty-two knees (4.5%) were revised to a total knee arthroplasty. Reasons for revision were aseptic loosening of the femoral (n=8) or tibial component (n=2), undiagnosed pain (n=5), patellofemoral pain (n=2), infection (n=1), lateral meniscus tear (n=1), periprosthetic fracture (n=1), joint instability (n=1), and dislocation of meniscal bearing (n=1). Mean time to revision surgery from the primary procedure was 3.0 yrs. Eight-year survival for the cohort was 87.4%. The median pre-operative Oxford knee score was 62.5% which reduced to 27.7% at four years post-operatively. Conclusion. This study has demonstrated our revision rate for the Oxford unicompartmental knee replacement is comparable to independent series and national registry data. Post-operative function in patients not revised was good. The commonest reason for failure was aseptic component loosening which usually occurred within two to four years. Extended follow-up may therefore be beneficial in these patients so these cases are identified early and subsequently revised

Aims: Dislocation is one of the commonest complications following total hip arthroplasty. A significant proportion of these patients go on to have recurrent dislocations. Many factors have been identified as contributing to the risk of dislocation but treatment of recurrent dislocation is challenging. The use of semicaptive acetabular components is a potential solution to the problem of recurrent dislocation but there are few studies into the efficacy of these implants. Methods: Patients who underwent revision of their total hip arthroplasty to a semi-captive socket at the Lower Limb Arthroplasty Unit, Northern General Hospital, Sheffield between 2001 and March 2006 were studied. A proforma was designed and data was obtained from patients’ medical records. The number of dislocations both prior to revision to a semi-captive component and the following revision to a semi-captive acetabular component were recorded including the method of reduction Reasons for revision and the number and reasons for previous revisions were noted. Results: Average time of follow up was 22 months (range 2 months – 5 years 1 month). Following revision of total hip arthroplasty to a semi-captive component, 78% suffered no further dislocations. Of those patients who went on to redislocate, three went on to have recurrent dislocations and all were reduced by closed reduction. One patient redislocated and underwent a second revision to a semi-captive acetabular component, which was successful and underwent no further dislocations. Discussion. The results of this study show the use of semicaptive acetabular components in revision surgery for dislocation following total hip arthroplasty to be a highly effective solution to the problem of recurrent dislocation

Aims. Varus-valgus constrained (VVC) implants are often used during revision total knee arthroplasty (TKA) to gain coronal plane stability. However, the increased mechanical torque applied to the bone-cement interface theoretically increases the risk of aseptic loosening. We assessed mid-term survivorship, complications, and clinical outcomes of a fixed-bearing VVC device in revision TKAs. Methods. A total of 416 consecutive revision TKAs (398 patients) were performed at our institution using a single fixed-bearing VVC TKA from 2007 to 2015. Mean age was 64 years (33 to 88) with 50% male (199). Index revision TKA diagnoses were: instability (n = 122, 29%), aseptic loosening (n = 105, 25%), and prosthetic joint infection (PJI) (n = 97, 23%). All devices were cemented on the epiphyseal surfaces. Femoral stems were used in 97% (n = 402) of cases, tibial stems in 95% (n = 394) of cases; all were cemented. In total, 93% (n = 389) of cases required a stemmed femoral and tibial component. Femoral cones were used in 29%, and tibial cones in 40%. Survivorship was assessed via competing risk analysis; clinical outcomes were determined using Knee Society Scores (KSSs) and range of movement (ROM). Mean follow-up was four years (2 to 10). Results. The five-year cumulative incidence of subsequent revision for aseptic loosening and instability were 2% (95% confidence interval (CI) 0.2 to 3, number at risk = 154) and 4% (95% CI 2 to 6, number at risk = 153), respectively. The five-year cumulative incidence of any subsequent revision was 14% (95% CI 10 to 18, number at risk = 150). Reasons for subsequent revision included PJI (n = 23, of whom 12 had previous PJI), instability (n = 13), and aseptic loosening (n = 11). The use of this implant without stems was found to be a significant risk factor for subsequent revision (hazard ratio (HR) 7.58 (95% CI 3.98 to 16.03); p = 0.007). KSS improved from 46 preoperatively to 81 at latest follow-up (p < 0.001). ROM improved from 96° prerevision to 108° at latest follow-up (p = 0.016). Conclusion. The cumulative incidence of subsequent revision for aseptic loosening and instability was very low at five years with this fixed-bearing VVC implant in revision TKAs. Routine use of cemented and stemmed components with targeted use of metaphyseal cones likely contributed to this low rate of aseptic loosening. Cite this article: Bone Joint J 2020;102-B(4):458–462

Aims

Unicompartmental knee arthroplasty (UKA) has a higher risk of revision than total knee arthroplasty (TKA), particularly for younger patients. The outcome of knee arthroplasty is typically defined as implant survival or revision incidence after a defined number of years. This can be difficult for patients to conceptualize. We aimed to calculate the ‘lifetime risk’ of revision for UKA as a more meaningful estimate of risk projection over a patient’s remaining lifetime, and to compare this to TKA.

Methods

Incidence of revision and mortality for all primary UKAs performed from 1999 to 2019 (n = 13,481) was obtained from the New Zealand Joint Registry (NZJR). Lifetime risk of revision was calculated for patients and stratified by age, sex, and American Society of Anesthesiologists (ASA) grade.

Purpose: Pilon fractures demonstrate complex osseous and soft tissue injury. Protocols involving immediate tibial reduction and external fixation, with or without fibular fixation, then delayed definitive fixation result in decreased complications. Our purpose was to evaluate the treatment course of pilon fractures provisionally stabilised at outside institutions and subsequently transferred, focusing on the incidence and reasons for revision procedures, and subsequent complication rates. Method: An institutional trauma database was retrospectively reviewed, demonstrating 668 pilon fractures treated at our institution between 2000–2007. Of these, 39 patients with 42 fractures had a temporising surgical procedure prior to referral. Demographics, injury characteristics, reason for revision, and subsequent complications were determined. Clinical follow-up averaged 60 weeks (range, 1 to 281). Results: Mean age was 41 years (range, 18–78). Twenty-two fractures (52%) were open; 38 (90%) demonstrated a fractured fibula. Referral occurred an average of 5.8 days (range, 1–20) after initial stabilization. Pre-transfer fixation was revised in 40 fractures (95%). Reasons for revision included tibial malreduction (33 fractures, 83%), fibular malreduction (4 fractures, 10%), pins in the proposed incision (5 fractures, 13%), or loose pins (3 fractures, 8%). Of the 34 fractures with distal pins, 24 (71%) required revision for pin malposition, loosening, drainage, talar placement, or extraosseous placement. Late complications occurred in 14 fractures (33%), including deep infection in 10 (24%), and non-union in 3 (7%). Twenty-three patients (55%) required additional procedures following definitive fixation, including 9 soft tissue coverage procedures and 3 amputations. Conclusion: The majority of patients with pilon fractures treated with provisional stabilisation followed by referral to our institution required revision prior to definitive fixation. This resulted in many avoidable additional procedures, and a higher complication rate than recent contemporary controls. The authors recommend that, when possible, the initial and definitive management of these injuries be performed at the accepting institution

Aim: To study of the results of the first 100 Miller-Galante, unicompartmental knee replacements performed by one surgeon over the period from May 1990 to November 1996, with particular reference to the failure mechanisms. Method: A cohort of 100 patients has been followed up prospectively on a biennial basis. They have been assessed using the Nottingham Knee Proforma of Tew & Waugh. An up-dated review was carried out of the cases for the preparation of this presentation. Results:. Alive, knee functioning satisfactorily – 57. Dead, knee functioning satisfactorily – 21. Lost to follow up – 10. Significant symptoms, not revised – 2. Infection – 1 case not requiring revision. Revised – 9. Due for revision – 1. Of the 32 patients over the age of 74 at the time of replacement none have required revision (18 still alive). Reasons for revision:. 6 developed degeneration in another compartment. 2 had unexplained pain. 1 developed loosening of the tibial component (19 components found to be well fixed to bone at revision). 1 developed significant wear and is due for revision. Conclusions: This prosthesis remains a surgical option for treatment of unicompartmental knee osteoarthrosis. As judged on survival it appears to be superior to high tibial osteotome (HTO) but it appears to be inferior to total knee replacement (TKR). It is easier to revise than failed HTO or TKR. Careful patient selection is obviously required

Introduction. Modularity is being increasingly used throughout the world for both primary and revision total hip arthroplasty. Recently there have been concerns of increased corrosion and fretting at the modular junctions. In the SROM® modular hip system, two modular junctions are the head-neck taper junction and the stem-sleeve taper junction. The aim of this study was to investigate corrosion at these junctions with the use of different bearing materials. Methods. Between 1994 and 2012, fourty-two patients were revised with SROM® stems. Reasons for revision included aseptic loosening of the cup or stem (11), periprosthetic fracture (2), osteolysis (8), dislocation (13) and other reasons (7). One was revised for stem breakage, and this was excluded from this study. We examined 41 retrieved S-ROM® comprised of 6 metal-on-metal (MOM), 12 metal-on-polyethylene (MOP), 7 ceramic-on-polyethylene (COP) and 16 ceramic-on-ceramic (COC). The orientation for all components was marked at the time of revision surgery. Both the proximal sleeve/stem and the femoral head-neck modular junctions were examined under 10X magnification, and graded by two independent observers. The head tapers were divided into 4 regions, and graded using a previously published 3 point scoring system for fretting and corrosion damage (Goldberg et al, Kop et al), for a total corrosion damage score of 12. The SROM stems were also assessed at the sleeve/stem taper junction. Each stem was divided into 8 quadrants, and graded for corrosion and fretting using the same system as the taper. In addition to severity, we also quantified area of corrosion damage of the stem at the sleeve-stem junction from 0–3, which was multiplied by the severity of damage, to give a score out of 9 for each quadrant (maximum total score of 72 for the stem). The bearing type was unknown to the investigators, so the grading was done in a blinded fashion. Corrosion scores were divided by time to account for differences in time to revision. Results. Corrosion at the head-neck taper junction was higher for 17 stems with metal heads compared to 23 stems with ceramic heads (p=0.008). The average corrosion rate at the stem-sleeve taper junction in hips with hard-on-hard bearings (COC and MOM) was higher than polyethylene (MOP and COP) bearings, but this was not significant (p=0.07). Conclusions. Corrosion at the head-neck modular junction of hips with metal heads was greater than ceramic heads, likely due to galvanic corrosion in a mixed-metal system. Greater corrosion was found at the stem-sleeve taper junction in stems with hard-on-hard bearings. This may be related to friction in the bearings

Aims

Despite multiple trials and case series on hip hemiarthroplasty designs, guidance is still lacking on which implant to use. One particularly deficient area is long-term outcomes. We present over 1,000 consecutive cemented Thompson’s hemiarthroplasties over a ten-year period, recording all accessible patient and implant outcomes.

Methods

Patient identifiers for a consecutive cohort treated between 1 January 2003 and 31 December 2011 were linked to radiographs, surgical notes, clinic letters, and mortality data from a national dataset. This allowed charting of their postoperative course, complications, readmissions, returns to theatre, revisions, and deaths. We also identified all postoperative attendances at the Emergency and Outpatient Departments, and recorded any subsequent skeletal injuries.

Summary. The M2a-38. tm. metal on metal total hip arthroplasty showed a high incidence of pseudotumors and an unexpected high revision rate in our thoroughly screened cross sectional cohort. Introduction. After the revival of the metal on metal (MoM) bearing in total hip arthroplasty (THA) at the beginning of this century, there are now serious questions about this type of bearing. The advantage of large head MoM bearing is the increase in range of motion and stability. In our institution the choice was made for 38 mm heads. During the last few years concerns have been raised about the relationship of MoM bearing and elevated serum cobalt and chromium ion levels, their local and systemic toxicological effects and the incidence of local tumorous masses (pseudotumors). Are these findings applicable for all MoM bearings or are there also product specific issues. We present the outcome of a cementless MoM THA using a 38mm head in a unique consecutive series of 377 THA who were performed in our institution. Patients and Methods. All 351 patients (377 THA) with a cementless MoM THA (M2a-38. tm. , Biomet Inc, Warsaw, IN, USA, and Taperloc® stem, Biomet UK, Bridgend) between 2008 and 2011 were evaluated. All patients were analyzed by a physical exam, serum levels of cobalt and chromium and an interview to determine if there were any complaints. An MRI of the hip was made if patients reported pain during physical activity, allergies to metals, serum cobalt or chromium ion levels ≥ 5 ppb or if the inclination of the acetabular component was more than 50 degrees. Nine patients deceased, three were lost to follow up and four already underwent a revision before the screening. We analyzed 361 hips with an average follow up of 30 (range 2–58) months. The average preoperative age was 63 years (41–88). Results. 219 patients with 235 THA (65%) reported no complaints. Median cobalt level in patients with complaints was 6.6 (0.2–173) ppb and in the group without complaints 3.7 (0.2–27.3) ppb. Median chromium level in patients with complaints was 5.0 (0.1–134) ppb and in the group without complaints 3.7 (0.2–27) ppb. On the 226 performed MRI scans, 56 pseudotumors were diagnosed and described using the Anderson classification (9 C1, 41 C2 and 5 C3). 71 hips had been revised after a mean follow of 30 months (range 0.2–50 months). Reasons for revision were because of pain, raised metal ions and a pseudotumor in 28; pain and raised metal ion levels in 15; aseptic loosening of the acetabular or femoral component in 11; raised metal ions and a pseudotumor in 7; combination of luxations, luxation feelings and fractures in 5; infections in 3 and for other reasons in 2. Conclusion. The short-term results of the THA with a 38mm head metal on metal articulation are higher compared with other MoM bearings. They show a revision rate of 7% and 10% in the Australian and England register. In other peer reviewed literature we find a revision rate between 0–13% after five years. We also observed a high incidence of elevated serum levels of metal ions, pseudotumors and an unexpected high early revision rate

Aims

The Exeter V40 femoral stem is the most implanted stem in the National Joint Registry (NJR) for primary total hip arthroplasty (THA). In 2004, the 44/00/125 stem was released for use in ‘cement-in-cement’ revision cases. It has, however, been used ‘off-label’ as a primary stem when patient anatomy requires a smaller stem with a 44 mm offset. We aimed to investigate survival of this implant in comparison to others in the range when used in primary THAs recorded in the NJR.

Methods

We analyzed 328,737 primary THAs using the Exeter V40 stem, comprising 34.3% of the 958,869 from the start of the NJR to December 2018. Our exposure was the stem, and the outcome was all-cause construct revision. We stratified analyses into four groups: constructs using the 44/00/125 stem, those using the 44/0/150 stem, those including a 35.5/125 stem, and constructs using any other Exeter V40 stem.

Wear of the polyethylene (PE) insert in total knee replacements can lead to wear-particle and fluid-pressure induced osteolysis. One major factor affecting the wear behaviour of the PE insert in-vivo is the surface characteristics of the articulating femoral components. Contemporary femoral components available in Canada are either made of cast Cobalt Chromium (CoCr) alloy or have an oxidized zirconium surface (Oxinium). The latter type of femoral components have shown to have increased abrasive wear resistance and increased surface wettability, thus leading to reduced PE wear in-vitro compared with conventional cast CoCr components. Although surface damage has been reported on femoral components in general, there have been no reports in the literature as to what extent the recommended operating techniques affect the surface tribology of either type of femoral component. Twenty-two retrieved total knee replacements were identified with profound surface damage on the posterior aspect of the femoral condyles. The femoral components were of three different knee systems: five retrievals from the NexGen(r) total knee system (Zimmer Inc., Warsaw, IN), twelve retrievals from the Genesis II(r) total knee system (CoCr alloy or Oxinium; Smith & Nephew Inc., Memphis, TN), and five retrievals from the Duracon(r) total knee system (Stryker Inc., Mahwah, NJ). Reasons for revision were all non-wear-related and included aseptic loosening in two cases, painful flexion instability, and chronic infection. All retrieved femoral components showed evidence of surface damage on the condyles, at an average of 99° flexion (range, 43° – 135° flexion). Titanium (Ti) alloy transfer and abrasive surface damage were evident on all retrieved CoCr alloy femoral components that came in contact with Ti alloy tibial trays. Surface damage on the retrieved Oxinium femoral components was gouging, associated with the removal and cracking of the oxide and exposure of the zirconium alloy substrate material. CoCr alloy femoral components that had unintended contact with CoCr alloy tibial trays also showed evidence of gouging and abrasive wear. All femoral components showed severe surface damage in the posterior aspect of the condyles. The femoral surface was heavily scratched and the oxidized zirconium coating surface appeared removed. The surface analysis suggested that the surface damage most likely occurred during the time of initial implantation. In particular, it appeared that the femoral condyles were resting on the posterior aspect of the tibial tray in flexion, thus scratching the femoral components. Such scratches could potentially lead to accelerated PE insert wear and reduced implant longevity, thus making expensive revisions surgery necessary. The authors strongly suggest a revision of the current operating techniques recommended by the implant manufacturer to prevent this type of surface damage from occurring

Methods

All TEAs recorded in the Dutch national registry between 2014 and 2020 were included. The Kaplan-Meier method was used for survival analysis, and a logistic regression model was used to assess the factors associated with revision.

Purpose:. Acetabular bone loss during revision total hip arthroplasty (THA) poses a challenge for reconstruction as segmental and extensive cavitary defects require structural support to achieve prosthesis stability. Trabecular metal (TM) acetabular augments structurally support hemispherical cups. Positive short-term results have been encouraging, but mid- to long-term results are largely unknown. The purpose of this study was to determine the continued efficacy of TM augments in THA revisions with significant pelvic bone loss. Methods:. Radiographs and medical records of 51 patients who had undergone THA revision with the use of a TM augment were retrospectively reviewed. Acetabular defects were graded according to the Paprosky classification of acetabular deficiencies based on preoperative radiographs and operative findings. Loosening was defined radiographically as a gross change in cup position, change in the abduction angle (>5°), or change in the vertical position of the acetabular component (>8 mm) between initial postoperative and most recent follow-up radiographs (Figure 1). Results:. Eleven patients had incomplete radiographic follow-up and were excluded. The study population included 17 men and 23 women, averaging 68.1 ± 14.1 years of age (range, 37–91), with average radiographic follow-up of 19.0 months (range, 2.4–97.4). Reasons for revision included osteolysis (n = 20, 38.5%), component loosening (n = 18, 15.4%), and periprosthetic fracture (n = 6, 11.5%). All patients underwent revision THA using a TM multi-hole revision acetabular cup and TM acetabular augment(s) to fill bony defects. Morcellized allograft was used in 9 patients. There were 33 Paprosky Type IIIA and seven Paprosky Type IIIB defects. One patient with Paprosky Type IIIB had catastrophic failure of the reconstructive construct three months postoperatively. The remaining 39 acetabular revisions demonstrated signs of bony ingrowth at the latest follow-up. There were no radiolucent lines suggestive of loosening, and no significant differences in abduction angle (p = 0.78), vertical distance between the superolateral edge of the cup and the trans-ischial reference line (p = 0.96), or the vertical distance between the center of the femoral head and trans-ischial reference line (p = 0.75) between the initial postoperative and most recent follow-up radiographs (Figure 2). Discussion and Conclusion:. Achieving fixation and long-term stability in THA revisions with segmental and/or cavitary bone loss is challenging. TM augments provide a modular structural system to achieve bony ingrowth, while avoiding large structural allografts, cages, and custom implants. At latest follow-up, 39 revision hips remained well-fixed with no evidence of loosening. One patient with a significant surgical history of infection, periprosthetic femur fracture, and 2 prior revision surgeries before acetabular reconstruction had an early clinical failure. Trabecular metal augments can be used for reconstruction of acetabular bone loss with good mid-term results. Continued follow-up is warranted for radiographic evaluation of bony integration and implant stability to determine long-term survivorship of these implants

Introduction: With an ever-increasing elderly population the rise in primary total hip replacement is inevitable. This translates into a comparative rise in revision hip surgery. We performed an observational study to determine current revision hip surgical practices in one UK region. Methods: We utilized a regional hip register (Trent Arthroplasty Audit Group (TAAG)) to review current revision hip surgical practice in our region. 136 surgeons in 21 different hospitals, covering a population of 6.2 million, contribute to the register. We analysed completed data forms to produce the following results. Results: 875 revision THRs performed between 2000 and 2002 were identified. 54% were female, with a mean age of 70 (range 26–97). 45 different femoral stems requiring revision were identified. Reasons for revision were aseptic loosening of both components (23%), cup alone (24%), stem alone (17%), infection (14%), recurrent dislocation (10%), periprosthetic fracture (5%) and acetabular erosion (3%). Over 25 different femoral stems and over 30 different acetabular components were utilised by surgeons. In 70% of cases the femoral and acetabular components were made by different manufacturers. Bone graft was used in approximately 50% of cases. 90% of centres contributing to TAAG perform revision hip surgery. 24% of all revision THRs were performed by surgeons performing 5 or less per year. Only 40% of revision THRs were performed by ‘revision surgeons’ performing > 20 per year. Conclusions: It is evident the majority of centres, both teaching and DGH, in our region regularly perform revision hip surgery, with up to a quarter performed by non-revision specialists. With increasing numbers of prostheses available, high numbers of component mismatch use, and the inevitable increase in future need for revision hip surgery, is the occasional revision surgeon compromising patient care? Will this have future medico-legal consequences?

Development of the metal/ metal hip resurfacing began in 1989 with the first patient implantation in February 1991. In the first three years three methods of implant fixation were employed in a pilot study of 119 resurfacings and the optimum fixation was identified as Hydroxyapatite Coated (HA) uncemented cups and cemented femoral components. From March 1994 until February 2001 the author has performed 1503 hybrid fixed resurfacing implants, 294 of the McMinn type (HA on Smooth Metal Cup) and 1209 Birmingham Hip resurfacings (BHR; HA on Porous Metal Cup). Mean age was 52.9 years, diagnosis – Osteoarthritis 78%; Dysplasia 7%; Inflammatory 2%; Avascular necrosis 5%; other 8%. In the 294 McMinn Resurfacings 8 have failed and 7 have been revised. Reasons for failure; 5 cup loosenings, two infections and one femoral head collapse in a patient with pre-existing avascular necrosis. High wear of the metal/ metal bearings, with altered metallurgy, is now presenting as a cause for concern with these McMinn resurfacings performed in 1996 where the metal was Solution Heat Treated (SHT) and Hot Isostatically Pressed (HIP). Patients from 1996 are exhibiting asymptomatic radiographic osteolysis and at revision metallosis is seen with high wear measured on the retrieved components. Laboratory studies have shown that SHT and HIP alters the carbide microstructure and increases wear on abrasive wear testing. In the 1209 BHR’s 7 failures have occurred requiring revision. Reasons for revision – 4 femoral neck fractures, 2 infections and 1 collapsed femoral head from avascular necrosis. Implant fixation is reliable as judged by very low migration on an R.S.A. study. Bearing wear is very low as judged from wear analysis of explants as would be expected from the historically proven “As Cast” metal microstructure. Function is good as judged by the Harris Hip Score (mean 96.3 at 3years). Cumulative survival on all Hybrid fixed implants is 97% at 7–8 years. Hip resurfacing, using metal on metal bearings with historically proven metallurgy using hybrid fixation, offers a viable treatment alternative for the young and more active patient with hip arthritis

Aims

Dual-mobility (DM) components are increasingly used to prevent and treat dislocation after total hip arthroplasty (THA). Intraprosthetic dissociation (IPD) is a rare complication of DM that is believed to have decreased with contemporary implants. This study aimed to report incidence, treatment, and outcomes of contemporary DM IPD.

Methods

A total of 1,453 DM components were implanted at a single academic institution between January 2010 and December 2021: 695 in primary and 758 in revision THA. Of these, 49 presented with a dislocation of the large DM head and five presented with an IPD. At the time of closed reduction of the large DM dislocation, six additional IPDs occurred. The mean age was 64 years (SD 9.6), 54.5% were female (n = 6), and mean follow-up was 4.2 years (SD 1.8). Of the 11 IPDs, seven had a history of instability, five had abductor insufficiency, four had prior lumbar fusion, and two were conversions for failed fracture management.

Methods

De-identified individual-level data on all revision knee arthroplasties performed in Australia from 2007 to 2017 were obtained from the Australian Orthopaedic Association National Joint Replacement Registry. Population data and life tables were obtained from the Australian Bureau of Statistics. The lifetime risk of revision surgery was calculated for each year using a standardized formula. Separate calculations were undertaken for males and females.

Aims

The aim of this study was to evaluate the diagnostic accuracy of the absolute synovial polymorphonuclear neutrophil cell (PMN) count for the diagnosis or exclusion of periprosthetic joint infection (PJI) after total hip (THA) or knee arthroplasty (TKA).

Methods

In this retrospective cohort study, 147 consecutive patients with acute or chronic complaints following THA and TKA were included. Diagnosis of PJI was established based on the 2018 International Consensus Meeting criteria. A total of 39 patients diagnosed with PJI (32 chronic and seven acute) and 108 patients with aseptic complications were surgically revised.

Aims

The aim of this study was to conduct a cross-sectional, observational cohort study of patients presenting for revision of a total hip, or total or unicompartmental knee arthroplasty, to understand current routes to revision surgery and explore differences in symptoms, healthcare use, reason for revision, and the revision surgery (surgical time, components, length of stay) between patients having regular follow-up and those without.

Methods

Data were collected from participants and medical records for the 12 months prior to revision. Patients with previous revision, metal-on-metal articulations, or hip hemiarthroplasty were excluded. Participants were retrospectively classified as ‘Planned’ or ‘Unplanned’ revision. Multilevel regression and propensity score matching were used to compare the two groups.

Aims

Wear of the polyethylene (PE) tibial insert of total knee arthroplasty (TKA) increases the risk of revision surgery with a significant cost burden on the healthcare system. This study quantifies wear performance of tibial inserts in a large and diverse series of retrieved TKAs to evaluate the effect of factors related to the patient, knee design, and bearing material on tibial insert wear performance.

Methods

An institutional review board-approved retrieval archive was surveyed for modular PE tibial inserts over a range of in vivo duration (mean 58 months (0 to 290)). Five knee designs, totalling 1,585 devices, were studied. Insert wear was estimated from measured thickness change using a previously published method. Linear regression statistical analyses were used to test association of 12 patient and implant design variables with calculated wear rate.

Aims

The aim of our study was to investigate the effect of asymmetric crosslinked polyethylene liner use on the risk of revision of cementless and hybrid total hip arthroplasties (THAs).

Methods

We undertook a registry study combining the National Joint Registry dataset with polyethylene manufacturing characteristics as supplied by the manufacturers. The primary endpoint was revision for any reason. We performed further analyses on other reasons including instability, aseptic loosening, wear, and liner dissociation. The primary analytic approach was Cox proportional hazard regression.

Aims

Uncemented mobile bearing designs in medial unicompartmental knee arthroplasty (UKA) have seen an increase over the last decade. However, there are a lack of large-scale studies comparing survivorship of these specific designs to commonly used cemented mobile and fixed bearing designs. The aim of this study was to evaluate the survivorship of these designs.

Methods

A total of 21,610 medial UKAs from 2007 to 2018 were selected from the Dutch Arthroplasty Register. Multivariate Cox regression analyses were used to compare uncemented mobile bearings with cemented mobile and fixed bearings. Adjustments were made for patient and surgical factors, with their interactions being considered. Reasons and type of revision in the first two years after surgery were assessed.

Aims

Despite few good-quality studies on the subject, total hip arthroplasty (THA) is increasingly being performed for displaced intracapsular fractures of the neck of femur. We compared outcomes of all patients with displacement of these fractures treated surgically over a ten-year period in one institution.

Methods

A total of 2,721 patients with intracapsular fractures of the femoral neck treated with either a cemented hemiarthroplasty or a THA at a single centre were retrospectively reviewed. The primary outcomes analyzed were readmission for any reason and revision surgery. We secondarily looked at mortality rates.

Aims

Anterior cruciate ligament (ACL) and multiligament knee (MLK) injuries increase the risk of development of knee osteoarthritis and eventual need for total knee arthroplasty (TKA). There are limited data regarding implant use and outcomes in these patients. The aim of this study was to compare the use of constrained implants and outcomes among patients undergoing TKA with a history of prior knee ligament reconstruction (PKLR) versus a matched cohort of patients undergoing TKA with no history of PKLR.

Patients and Methods

Patients with a history of ACL or MLK reconstruction who underwent TKA between 2007 and 2017 were identified in a single-institution registry. There were 223 patients who met inclusion criteria (188 ACL reconstruction patients, 35 MLK reconstruction patients). A matched cohort, also of 223 patients, was identified based on patient age, body mass index (BMI), sex, and year of surgery. There were 144 male patients and 79 female patients in both cohorts. Mean age at the time of TKA was 57.2 years (31 to 88). Mean BMI was 29.7 kg/m2 (19.5 to 55.7).

Aims

The purpose of this study is to examine six types of bearing surfaces implanted at a single institution over three decades to determine whether the reasons for revision vary among the groups and how long it takes to identify differences in survival.

Methods

We considered six cohorts that included a total of 1,707 primary hips done between 1982 and 2010. These included 223 conventional polyethylene sterilized with γ irradiation in air (CPE-GA), 114 conventional polyethylene sterilized with gas plasma (CPE-GP), 116 crosslinked polyethylene (XLPE), 1,083 metal-on-metal (MOM), 90 ceramic-on-ceramic (COC), and 81 surface arthroplasties (SAs). With the exception of the COC, all other groups used cobalt-chromium (CoCr) femoral heads. The mean follow-up was 10 (0.008 to 35) years. Descriptive statistics with revisions per 100 component years (re/100 yr) and survival analysis with revision for any reason as the endpoint were used to compare bearing surfaces.

Aims

Femoral revision component subsidence has been identified as predicting early failure in revision hip surgery. This comparative cohort study assessed the potential risk factors of subsidence in two commonly used femoral implant designs.

Methods

A comparative cohort study was undertaken, analyzing a consecutive series of patients following revision total hip arthroplasties using either a tapered-modular (TM) fluted titanium or a porous-coated cylindrical modular (PCM) titanium femoral component, between April 2006 and May 2018. Clinical and radiological assessment was compared for both treatment cohorts. Risk factors for subsidence were assessed and compared.

Aims

It remains controversial whether patellofemoral joint pathology is a contraindication to lateral unicompartmental knee arthroplasty (UKA). This study aimed to evaluate the effect of preoperative radiological degenerative changes and alignment on patient-reported outcome scores (PROMs) after lateral UKA. Secondarily, the influence of lateral UKA on the alignment of the patellofemoral joint was studied.

Methods

A consecutive series of patients who underwent robotic arm-assisted fixed-bearing lateral UKA with at least two-year follow-up were retrospectively reviewed. Radiological evaluation was conducted to obtain a Kellgren Lawrence (KL) grade, an Altman score, and alignment measurements for each knee. Postoperative PROMs were assessed using the Kujala (Anterior Knee Pain Scale) score, Knee Injury and Osteoarthritis Outcome Score Joint Replacement (KOOS JR), and satisfaction levels.

Introduction: Loosening of acetabular components often lead to excessive bone defects. Managing severe acetabular bone loss in revision arthroplasty is a serious or sometimes even an impossible challenge. Several authors even have published disappointing results. The purpose of this study was to evaluate the long term clinical and radiographic results of acetabular reconstruction with the use of bone impaction grafting and a cemented cup in cases with very large acetabular defects. Methods: In this historical prospective study, 27 hips (25 patients) with large acetabular defects (AAOS type III and IV, Paprosky type 2B to 3B) were reviewed. In all cases an extensive reconstruction with the use of bone impaction grafting and a large rim wire mesh (Stryker Howmedica, Newbury, UK) was performed. All patients have had a six week bed rest period or 3 weeks of bed rest with 3 weeks of non-weight bearing mobilization. No patient was lost to follow-up. Three patients died during follow-up, the cause of death was in all cases not related to the surgery. All medical files and radiographs were analyzed. Results: After a mean follow-up of 8.8 years (range 4 to 14.1), 3 hips needed repeat revision and another 2 hips were considered radiographic loose. Reasons for repeat revision were: septic loosening after 4.7 years, aseptic/traumatic loosening after 5.8 years and a broken-out reconstruction after 5 weeks because lack of compliance of the patient. Kaplan-Meier analysis showed a ten-year survival rate of 87% (95%C.I. 73.1–100%) with end point acetabular revision for any reason and 95% (95%C.I. 86.2–100%) with end point acetabular revision because of aseptic loosening. The mean preoperative HHS was 55 points and improved to 72 points postoperative. Conclusion: Acetabular reconstruction using impaction bone grafting and a cemented cup can produce favorable long-term results in patients with massive acetabular bone-stock defects. It is the only revision technique which restores massive acetabular bone stock loss. However, in these extensive defects it is a technical demanding procedure and should only be used by surgeons with extensive experience with this technique

Aims: The purpose of this study was to determine the success of retaining a well-þxed acetabular component at the time of isolated femoral revision and identify variables that may inßuence clinical decision making and outcome. Methods: 83 consecutive patients (90 hips) with a retained cementless socket during revision of a femoral component were reviewed. Eighty-þve percent of the hips were initially reconstructed as metal on polyethylene surface arthroplasty (SA) and 15% as a standard stem type total hip replacement (THR). Average age at the time of revision was 54. 33% of the patients had acetabular osteolysis of which 52% were grafted. Results: Mean follow up 9.6 years (5.5 to 15.9). Average time in situ of socket 14.8 years (7.1 to 20.2). 5 sockets were revised at a mean of 6.8 (2.5–10.4) years post revision and 13.1 years in situ. Reasons for socket revision were: 1 socket loosening, 1 dissociation of the liner cemented into the original socket, 1 instability, 1 conversion to a Metal/Metal bearing and 1 because it was the surgeonñs preference to remove the well-þxed socket. Dislocation rate was 15% with 50% occurring in the þrst 6 months post revision. Nine had more than one episode of dislocation. No recurrence or expansion of pelvic osteolysis was observed at last follow-up. Discussion: Revision of a cementless socket on the basis of the duration in vivo and/or presence of osteolysis does not appear to be warranted. Because of the high risk of dislocation off set liners/stems and larger femoral head sizes should be considered, especially in revision of SA

Objectives

The Attune total knee arthroplasty (TKA) has been used in over 600 000 patients worldwide. Registry data show good clinical outcome; however, concerns over the cement-tibial interface have been reported. We used retrieval analysis to give further insight into this controversial topic.

Methods

We examined 12 titanium (Ti) PFC Sigma implants, eight cobalt-chromium (CoCr) PFC Sigma implants, eight cobalt-chromium PFC Sigma rotating platform (RP) implants, and 11 Attune implants. We used a peer-reviewed digital imaging method to quantify the amount of cement attached to the backside of each tibial tray. We then measured: 1) the size of tibial tray thickness, tray projections, peripheral lips, and undercuts; and 2) surface roughness (Ra) on the backside and keel of the trays. Statistical analyses were performed to investigate differences between the two designs.

A total of 558 Avon PFJ arthroplasties in 431 patients, with minimum two-year follow-up, were identified from a prospective database. Patient-reported outcomes and implant survivorship were analyzed, with follow-up of up to 18 years.

Objectives

The use of ceramic femoral heads in total hip arthroplasty (THA) has increased due to their proven low bearing wear characteristics. Ceramic femoral heads are also thought to reduce wear and corrosion at the head-stem junction with titanium (Ti) stems when compared with metal heads. We sought to evaluate taper damage of ceramic compared with metal heads when paired with cobalt chromium (CoCr) alloy stems in a single stem design.

Methods

This retrieval study involved 48 total hip arthroplasties (THAs) with CoCr V40 trunnions paired with either CoCr (n = 21) or ceramic (n = 27) heads. The taper junction of all hips was evaluated for fretting/corrosion damage and volumetric material loss using a roundness-measuring machine. We used linear regression analysis to investigate taper damage differences after adjusting for potential confounding variables.

Aims

We aimed to assess the comparability of data in joint replacement registries and identify ways of improving the comparisons between registries and the overall monitoring of joint replacement surgery.

Materials and Methods

We conducted a review of registries that are full members of the International Society of Arthroplasty Registries with publicly available annual reports in English. Of the six registries which were included, we compared the reporting of: mean age, definitions for revision and re-operation, reasons for revision, the approach to analysing revisions, and patient-reported outcome measures (PROMs) for primary and revision total hip arthroplasty (THA) and hip resurfacing arthroplasty (HRA).

The aim of this study was to analyse the gait pattern, muscle force and functional outcome of patients who had undergone replacement of the proximal tibia for tumour and alloplastic reconstruction of the extensor mechanism using the patellar-loop technique.

Between February 1998 and December 2009, we carried out wide local excision of a primary sarcoma of the proximal tibia, proximal tibial replacement and reconstruction of the extensor mechanism using the patellar-loop technique in 18 patients. Of these, nine were available for evaluation after a mean of 11.6 years (0.5 to 21.6). The strength of the knee extensors was measured using an Isobex machine and gait analysis was undertaken in our gait assessment laboratory. Functional outcome was assessed using the American Knee Society (AKS) and Musculoskeletal Tumor Society (MSTS) scores.

The gait pattern of the patients differed in ground contact time, flexion heel strike, maximal flexion loading response and total sagittal plane excursion. The mean maximum active flexion was 91° (30° to 110°). The overall mean extensor lag was 1° (0° to 5°). The mean extensor muscle strength was 25.8% (8.3% to 90.3%) of that in the non-operated leg (p < 0.001). The mean functional scores were 68.7% (43.4% to 83.3%) (MSTS) and 71.1 (30 to 90) (AKS functional score).

In summary, the results show that reconstruction of the extensor mechanism using this technique gives good biomechanical and functional results. The patients’ gait pattern is close to normal, except for a somewhat stiff knee gait pattern. The strength of the extensor mechanism is reduced, but sufficient for walking.

Despite excellent results, the use of cemented total hip replacement (THR) is declining. This retrospective cohort study records survival time to revision following primary cemented THR using the most common combination of components that accounted for almost a quarter of all cemented THRs, exploring risk factors independently associated with failure. All patients with osteoarthritis who had an Exeter V40/Contemporary THR (Stryker) implanted before 31 December 2010 and recorded in the National Joint Registry for England and Wales were included in the analysis. Cox’s proportional hazard models were used to analyse the extent to which risk of revision was related to patient, surgeon and implant covariates, with a significance threshold of p < 0.01. A total of 34 721 THRs were included in the study. The overall seven-year rate of revision for any reason was 1.70% (99% confidence interval (CI) 1.28 to 2.12). In the final adjusted model the risk of revision was significantly higher in THRs with the Contemporary hooded component (hazard ratio (HR) 1.88, p < 0.001) than with the flanged version, and in smaller head sizes (< 28 mm) compared with 28 mm diameter heads (HR 1.50, p = 0.005). The seven-year revision rate was 1.16% (99% CI 0.69 to 1.63) with a 28 mm diameter head and flanged component. The overall risk of revision was independent of age, gender, American Society of Anesthesiologists grade, body mass index, surgeon volume, surgical approach, brand of cement/presence of antibiotic, femoral head material (stainless steel/alumina) and stem taper size/offset. However, the risk of revision for dislocation was significantly higher with a ‘plus’ offset head (HR 2.05, p = 0.003) and a hooded acetabular component (HR 2.34, p < 0.001).

In summary, we found that there were significant differences in failure between different designs of acetabular component and sizes of femoral head after adjustment for a range of covariates.

We analysed data from the Oxford hip and knee questionnaires collected by the New Zealand Joint Registry at six months and five years after joint replacement, to determine if there was any relationship between the scores and the risk of early revision. Logistic regression of the six-month scores indicated that for every one-unit decrease in the Oxford score, the risk of revision within two years increased by 9.7% for total hip replacement (THR), 9.9% for total knee replacement (TKR) and 12.0% for unicompartmental knee replacement (UKR). Our findings showed that 70% of the revisions within two years for TKR and 67% for THR and UKR would have been captured by monitoring the lowest 22%, 28% and 28%, respectively, of the Oxford scores. When analysed using the Kalairajah classification a score of < 27 (poor) was associated with a risk of revision within two years of 7.6% for THR, 7.0% for TKR and 24.3% for UKR, compared with risks of 0.7%, 0.7% and 1.8%, respectively, for scores > 34 (good or excellent).

Our study confirms that the Oxford hip and knee scores at six months are useful predictors of early revision after THR and TKR and we recommend their use for the monitoring of the outcome and potential failure in these patients.

We evaluated the impact of pre-coating the tibial component with polymethylmethacrylate (PMMA) on implant survival in a cohort of 16 548 primary NexGen total knee replacements (TKRs) in 14 113 patients. In 13 835 TKRs a pre-coated tray was used while in 2713 TKRs the non-pre-coated version of the same tray was used. All the TKRs were performed between 2001 and 2009 and were cemented. TKRs implanted with a pre-coated tibial component had a lower cumulative survival than those with a non-pre-coated tibial component (p = 0.01). After adjusting for diagnosis, age, gender, body mass index, American Society of Anesthesiologists grade, femoral coupling design, surgeon volume and hospital volume, pre-coating was an independent risk factor for all-cause aseptic revision (hazard ratio 2.75, p = 0.006). Revision for aseptic loosening was uncommon for both pre-coated and non-pre-coated trays (rates of 0.12% and 0%, respectively). Pre-coating with PMMA does not appear to be protective of revision for this tibial tray design at short-term follow-up.

Between 1999 and 2005, 10 264 patients who had undergone total hip replacement (THR) for subcapital fracture of the hip were compared with 76 520 in whom THR had been performed for other reasons. All the cases were identified through the Swedish Hip Arthroplasty Register. The THRs performed as primary treatment for fracture were also compared with those done after failure of internal fixation.

After seven years the rate of revision was higher in THR after fracture (4.4% vs 2.9%). Dislocation and periprosthetic fracture were the most common causes of revision. The risk was higher in men than in women. The type of femoral component and the surgical approach influenced the risk. After correction for gender, type of component and the surgical approach the revision rates were similar in the primary and secondary fracture THR groups.

Total hip replacement is therefore a safe method for both the primary and secondary management of fracture of the hip.

We present a comparison of patient-reported outcomes (PROMs) in relation to patient age, in patients who had received a total (TKR) or unicompartmental knee replacement (UKR). The outcome was evaluated using the Oxford knee score (OKS), EuroQol (EQ-5D) and satisfaction scores. Patients aged 65 to 84 years demonstrated better pre-operative function scores than those aged < 65 years (OKS, p = 0.03; EQ-5D, p = 0.048) and those aged ≥ 85 years (OKS, p = 0.03). Post-operative scores were comparable across age groups, but a linear trend for greater post-operative improvement in OKS and EQ-5D was seen with decreasing age (p < 0.033). The overall mean satisfaction score at six months was 84.9, but those aged <  55 years exhibited a lower mean level of satisfaction (78.3) compared with all other age groups (all p < 0.031). The cumulative overall two-year revision rate was 1.3%. This study demonstrates that good early outcomes, as measured by the OKS and EQ-5D, can be anticipated following knee replacement regardless of the patient’s age, although younger patients gain greater improvement. However, the lower satisfaction in those aged < 55 years is a concern, and suggests that outcome is not fully encapsulated by the OKS and EQ-5D evaluation, and raises the question whether the OKS alone is an appropriate measure of pain and function in younger, more active individuals.

Current analysis of unicondylar knee replacements (UKRs) by national registries is based on the pooled results of medial and lateral implants. Consequently, little is known about the differential performance of medial and lateral replacements and the influence of each implant type within these pooled analyses. Using data from the National Joint Registry for England and Wales (NJR) we aimed to determine the proportion of UKRs implanted on the lateral side of the knee, and their survival and reason for failure compared with medial UKRs. By combining information on the side of operation with component details held on the NJR, we were able to determine implant laterality (medial versus lateral) for 32 847 of the 35 624 unicondylar replacements (92%) registered before December 2010. Of these, 2052 (6%) were inserted on the lateral side of the knee. The rates of survival at five years were 93.1% (95% confidence interval (CI) 92.7 to 93.5) for medial and 93.0% (95% CI 91.1 to 94.9) for lateral UKRs (p = 0.49). The rates of failure remained equivalent after adjusting for patient age, gender, American Society of Anesthesiologists (ASA) grade, indication for surgery and implant design using Cox’s proportional hazards method (hazard ratio for lateral relative to medial replacement = 0.88 (95% CI 0.69 to 1.13); p = 0.32). Aseptic loosening/lysis and unexplained pain were the main reasons for revision in both groups, although the reasons did vary depending on whether a mobile- or a fixed-bearing design was used. At a maximum of eight years the mid-term survival rates of medial and lateral UKRs are similar.

The Norwegian Arthroplasty Register has shown that several designs of uncemented femoral stems give good or excellent survivorship. The overall findings for uncemented total hip replacement however, have been disappointing because of poor results with the use of metal-backed acetabular components. In this study, we exclusively investigated the medium-to long-term performance of primary uncemented metal-backed acetabular components.

A total of 9113 primary uncemented acetabular components were implanted in 7937 patients between 1987 and 2007. These were included in a prospective, population-based observational study. All the implants were modular and metal-backed with ultra-high-molecular-weight polyethylene liners. The femoral heads were made of stainless steel, cobalt-chrome (CoCr) alloy or alumina ceramic. In all, seven different designs of acetabular component were evaluated by the Kaplan-Meier survivorship method and Cox regression analysis.

Most acetabular components performed well up to seven years. When the endpoint was revision of the acetabular component because of aseptic loosening, the survival ranged between 87% and 100% at ten years. However, when the endpoint was revision for any reason, the survival estimates were 81% to 92% for the same implants at ten years. Aseptic loosening, wear, osteolysis and dislocation were the main reasons for the relatively poor overall performance of the acetabular components. Prostheses with alumina heads performed slightly better than those with stainless steel or CoCr alloy in subgroups.

Whereas most acetabular components performed well at seven years, the survivorship declined with longer follow-up. Fixation was generally good. None of the metal-backed uncemented acetabular components with ultra-high-molecular-weight polyethylene liners in our study had satisfactory long-term results because of high rates of wear, osteolysis, aseptic loosening and dislocation.

Dislocation is a common reason for revision following total hip replacement. This study investigated the relationship between the bearing surface and the risk of revision due to dislocation. It was based on 110 239 primary total hip replacements with a diagnosis of osteoarthritis collected by the Australian Orthopaedic Association National Joint Replacement Registry between September 1999 and December 2007. A total of 862 (0.78%) were revised because of dislocation. Ceramic-on-ceramic bearing surfaces had a lower risk of requiring revision due to dislocation than did metal-on-polyethylene and ceramic-on-polyethylene surfaces, with a follow-up of up to seven years. However, ceramic-on-ceramic implants were more likely to have larger prosthetic heads and to have been implanted in younger patients. The size of the head of the femoral component and age are known to be independent predictors of dislocation. Therefore, the outcomes were stratified by the size of the head and age.

There is a significantly higher rate of revision for dislocation in ceramic-on-ceramic bearing surfaces than in metal-on-polyethylene implants when smaller sizes (≤ 28 mm) of the head were used in younger patients (< 65 years) (hazard ratio = 1.53, p = 0.041) and also with larger (> 28 mm) and in older patients (≥ 65 years) (hazard ratio = 1.73, p = 0.016).

Between 1989 and 1992 we had 102 knees suitable for unicompartmental knee replacement (UKR). They were randomised to receive either a St Georg Sled UKR or a Kinematic modular total knee replacement (TKR). The early results demonstrated that the UKR group had less complications and more rapid rehabilitation than the TKR group. At five years there were an equal number of failures in the two groups but the UKR group had more excellent results and a greater range of movement.

The cases were reviewed by a research nurse at 8, 10 and 12 years after operation. We report the outcome at 15 years follow-up. A total of 43 patients (45 knees) died with their prosthetic knees intact. Throughout the review period the Bristol knee scores of the UKR group have been better and at 15 years 15 (71.4%) of the surviving UKRs and 10 (52.6%) of the surviving TKRs had achieved an excellent score. The 15 years survivorship rate based on revision or failure for any reason was 24 (89.8%) for UKR and 19 (78.7%) for TKR. During the 15 years of the review four UKRs and six TKRs failed.

The better early results with UKR are maintained at 15 years with no greater failure rate. The median Bristol knee score of the UKR group was 91.1 at five years and 92 at 15 years, suggesting little functional deterioration in either the prosthesis or the remainder of the joint. These results justify the increased use of UKR.

We identified 148 patients who had undergone a revision total knee replacement using a single implant system between 1990 and 2000. Of these 18 patients had died, six had developed a peri-prosthetic fracture and ten had incomplete records or radiographs. This left 114 with prospectively-collected radiographs and Bristol knee scores available for study. The height of the joint line before and after revision total knee replacement was measured and classified as either restored to within 5 mm of the pre-operative height or elevated if it was positioned more than 5 mm above the pre-operative height. The joint line was elevated in 41 knees (36%) and restored in 73 (64%).

Revision surgery significantly improved the mean Bristol knee score from 41.1 (sd 15.9) pre-operatively to 80.5 (sd 15) post-operatively (p < 0.001). At one year post-operatively both the total Bristol knee score and its functional component were significantly better in the restored group than in the elevated group (p < 0.01). Overall, revision from a unicondylar knee replacement required less use of bone graft, fewer component augments, restored the joint line more often and gave a significantly better total Bristol knee score (p < 0.02) and functional score (p < 0.01) than revision from total knee replacement.

Our findings show that restoration of the joint line at revision total knee replacement gives a significantly better result than leaving it unrestored by more than 5 mm. We recommend the greater use of distal femoral augments to help to achieve this goal.

We have reviewed 42 patients who had revision of metal-on-metal resurfacing procedures, mostly because of problems with the acetabular component. The revisions were carried out a mean of 26.2 months (1 to 76) after the initial operation and most of the patients (30) were female.

Malpositioning of the acetabular component resulted in 27 revisions, mostly because of excessive abduction (mean 69.9°; 56° to 98°) or insufficient or excessive anteversion. Seven patients had more than one reason for revision. The mean increase in the diameter of the component was 1.8 mm (0 to 4) when exchange was needed.

Malpositioning of the components was associated with metallosis and a high level of serum ions. The results of revision of the femoral component to a component with a modular head were excellent, but four patients had dislocation after revision and four required a further revision.

Early failure associated with adverse reactions to metal debris is an emerging problem after hip resurfacing but the exact mechanism is unclear. We analysed our entire series of 660 metal-on-metal resurfacings (Articular Surface Replacement (ASR) and Birmingham Hip Resurfacing (BHR)) and large-bearing ASR total hip replacements, to establish associations with metal debris-related failures. Clinical and radiological outcomes, metal ion levels, explant studies and lymphocyte transformation tests were performed. A total of 17 patients (3.4%) were identified (all ASR bearings) with adverse reactions to metal debris, for which revision was required. This group had significantly smaller components, significantly higher acetabular component anteversion, and significantly higher whole concentrations of blood and joint chromium and cobalt ions than asymptomatic patients did (all p < 0.001). Post-revision lymphocyte transformation tests on this group showed no reactivity to chromium or cobalt ions. Explants from these revisions had greater surface wear than retrievals for uncomplicated fractures. The absence of adverse reactions to metal debris in patients with well-positioned implants usually implies high component wear.

Surgeons must consider implant design, expected component size and acetabular component positioning in order to reduce early failures when performing large-bearing metal-on-metal hip resurfacing and replacement.

Patellofemoral joint degeneration is often considered a contraindication to medial unicompartmental knee replacement. We examined the validity of this preconception using information gathered prospectively on the intra-operative status of the patellofemoral joint in 824 knees in 793 consecutive patients who underwent Oxford unicondylar knee replacement for anteromedial osteoarthritis. All operations were performed between January 1998 and September 2005. A five-point grading system classified degeneration of the patellofemoral joint from none to full-thickness cartilage loss. A subclassification of the presence or absence of any full-thickness cartilage loss was subsequently performed to test selected hypotheses. Outcome was evaluated independently by physiotherapists using the Oxford and the American Knee Society Scores with a minimum follow-up of one year.

Full-thickness cartilage loss on the trochlear surface was observed in 100 of 785 knees (13%), on the medial facet of the patella in 69 of 782 knees (9%) and on the lateral facet in 29 of 784 knees (4%). Full-thickness cartilage loss at any location was seen in 128 knees (16%) and did not produce a significantly worse outcome than those with a normal or near-normal joint surface. The severity of the degeneration at any of the intra-articular locations also had no influence on outcome.

We concluded that, provided there is not bone loss and grooving of the lateral facet, damage to the articular cartilage of the patellofemoral joint to the extent of full-thickness cartilage loss is not a contraindication to the Oxford mobile-bearing unicompartmental knee replacement.

There have been several reports of good survivorship and excellent function at ten years with fixed-bearing unicompartmental knee replacement. However, little is known about survival beyond ten years.

From the Bristol database of over 4000 knee replacements, we identified 203 St Georg Sled unicompartmental knee replacements (174 patients) which had already survived ten years. The mean age of the patients at surgery was 67.1 years (35.7 to 85) with 67 (38.5%) being under 65 years at the time of surgery. They were reviewed at a mean of 14.8 years (10 to 29.4) from surgery to determine survivorship and function. There were 99 knees followed up for 15 years, 21 for 20 years and four for 25 years. The remainder failed, were withdrawn, or the patient had died.

In 58 patients (69 knees) the implant was in situ at the time of death. Revision was undertaken in 16 knees (7.9%) at a mean of 13 years (10.2 to 21.6) after operation. In seven knees (3.4%) this was for progression of arthritis, in three (1.5%) for wear of polyethylene, in four (2%) for tibial loosening, in two (1%) for fracture of the femoral component and in two (1%) for infection. Two knees (1%) were revised for more than one reason.

The mean Bristol knee score of the surviving knees fell from 86 (34 to 100) to 79 (42 to 100) during the second decade. Survivorship to 20 years was 85.9% (95% CI 82.9% to 88.9%) and at 25 years was 80% (95% CI 70.2% to 89.8%). Satisfactory survival of a fixed-bearing unicompartmental knee replacement can be achieved into the second decade and beyond.

We have analysed from the Finnish Arthroplasty Register the long-term survivorship of the 12 most commonly-used cemented implants between 1980 and 2005 in patients aged 55 years or older with osteoarthritis.

Only two designs of femoral component, the Exeter Universal and the Müller Straight femoral component had a survivorship of over 95% at ten years with revision for aseptic loosening as the endpoint. At 15 years of the femoral and acetabular component combinations, only the Exeter Universal/Exeter All-poly implant had a survival rate of over 90% with revision for aseptic loosening as the endpoint. In the subgroup of patients aged between 55 and 64 years, survivorship overall was less than 90% at ten years.

The variation in the long-term rates of survival of different cemented hip implants was considerable in patients aged 55 years or older. In those aged between 55 and 64 years, none of the cemented prostheses studied yielded excellent long-term survival rates (≥ 90% at 15 years).

This prospective study describes the complications and survival of the first 688 Phase 3 Oxford medial unicompartmental knee replacements implanted using a minimally-invasive technique by two surgeons and followed up independently. None was lost to follow-up. We had carried out 132 of the procedures more than five years ago. The clinical assessment of 101 of these which were available for review at five years is also presented.

Nine of the 688 knees were revised: four for infection, three for dislocation of the bearing and two for unexplained pain. A further seven knees (1%) required other procedures: four had a manipulation under anaesthesia, two an arthroscopy and one a debridement for superficial infection. The survival rate at seven years was 97.3% (95% confidence interval 5.3). At five years, 96% of the patients had a good or excellent American Knee Society score, the mean Oxford knee score was 39 and the mean flexion was 133°. This study demonstrates that the minimally-invasive Oxford unicompartmental knee replacement is a reliable and effective procedure.

We report the clinical and radiographic outcome of a consecutive series of 138 hydroxyapatite-coated total knee replacements with a mean follow-up of 11 years (10 to 13). The patients were entered into a prospective study and all living patients (76 knees) were evaluated. The Hospital for Special Surgery knee score was obtained for comparison with the pre-operative situation. No patient was lost to follow-up. Radiographic assessment revealed no loosening. Seven prostheses have been revised, giving a cumulative survival rate of 93% at 13 years. We believe this to be the longest follow-up report available for an hydroxyapatite-coated knee replacement and the first for this design of Insall-Burstein II knee.
