The diagnosis and treatment of hip disease in young adults has rapidly evolved over the past ten years. Despite the advancements of improved diagnostic skills and refinement of surgical techniques, the psychosocial impact hip disease has on the young adult has not yet been elucidated.

This observational study aimed to characterise the functional and psychosocial characteristics of a group of patients from our young hip clinic. 49 patients responded to a postal questionnaire which included the Oswestry Disability Index (ODI) and Hospital Anxiety and Depression Scale (HADS). Median age was 20 years (range 16-38) with a gender ratio of 2:1 (female: male). The most common diagnoses were Perthes' disease and developmental hip dysplasia. More than half of our patients had moderate to severe pain based on the Visual Analogue Scale (VAS) and at least a moderate disability based on the ODI. Thirty-two percent of patients were classified as having borderline to abnormal levels of depression and 49% of patients were classified as having borderline to abnormal levels of anxiety based on the HADS. Comparison of the ODI with the VAS and HADS anxiety and depression subscales showed a significant positive correlation (p<0.05). Multiple regression showed the ODI to be a significant predictor of the HADS anxiety and depression scores (regression coefficient 0.13, 95% confidence interval 0.06 to 0.21, p<0.05).

This study highlights the previously unrecognised psychosocial effects of hip disease in the young adult. A questionnaire which includes HADS may be of particular value in screening for depression and anxiety in young people with physical illness. This study also highlights that collaboration with psychologists and other health care providers may be required to achieve a multidisciplinary approach in managing these patients.

Discitis in childhood is rare. It can be difficult to diagnose, particularly in the younger child, often leading to a delay in initiation of appropriate treatment. It is not known whether it represents an infective or an inflammatory process.

Our aim was to review all cases treated at a regional children's hospital since the introduction of the departmental database. A retrospective review (64,058 cases), for the period 1990-2008 was performed. 12 cases were identified (3 male/9 female), with a biphasic age distribution; eight [mean 22 months old (12-32)] and four [mean 12 years old (11-13)]. Mean time to diagnosis from onset was 22 days, (5-49). Symptoms varied with age, no one less than 28 months complained of back pain, while all over 28 months did, to a varying degree. All the younger children presented primarily with a gait abnormality. 92% (11/12) were apyrexial on admission. WBC and CRP were normal in 83% (10/12). Venous blood cultures were negative in 89% (8/9). Only ESR was mildly raised, mean 30 (10-65). Radiographs showed loss of intervertebral disc height in 91% (10/11), earliest by 10 days following onset symptoms, mean 28 days. A technetium bone scan was performed in 42% (5/12) and an MRI of the lumbar spine, in 58% (7/12). All were positive for discitis. All occurred in the lumbar spine, 50% at L3/4. Antibiotics were used in 11/12 (92%), flucloxacillin alone in the majority 9/11. One had non-steroidal medication alone. No form of brace was used. Mean follow-up was 13.3 months (2-36). In all, symptoms had resolved by mean 6.5 weeks (2-12). No recurrence was noted.

The common features of childhood discitis are presented; knowledge of these may aid the physician to come to a more rapid diagnosis of this uncommon paediatric condition.

Patients undergoing total knee arthroplasty (TKA) experience significant post-operative pain. We report the results of a new comprehensive patient care plan to manage peri-operative pain, enable early mobilisation and reduce length of hospital stay in TKA.

A prospective audit of 1081 patients undergoing primary TKA during 2008 and 2009 was completed. All patients followed a planned programme including pre-operative patient education, pre-emptive analgesia, spinal/epidural anaesthesia with propofol sedation, intra-articular soft tissue wound infiltration, post-operative high volume intermittent ropivacaine boluses with an intra-articular catheter and early mobilisation. The primary outcome measure was the day of discharge from hospital. Secondary outcomes were verbal rating pain scores on movement, time to first mobilisation, nausea and vomiting scores, urinary catheterisation for retention, need for rescue analgesia, maximum flexion at discharge and six weeks post-operatively, and Oxford score improvement.

The median day of discharge to home was post-operative day four. Median pain score on mobilisation was three for first post-operative night, day one and day two. 35% of patients ambulated on the day of surgery and 95% of patients within 24 hours. 79% patients experienced no nausea or vomiting. Catheterisation rate was 6.9%. Rescue analgesia was required in 5% of cases. Median maximum flexion was 85° on discharge and 93° at six weeks post-operatively. Only 6.6% of patients had a reduction in maximum flexion (loss of more than 5°) at six weeks. Median Oxford score had improved from 42 pre-operatively to 27 at six weeks post-operatively. The infection rate was 0.7% and the DVT and PTE rates were 0.6% and 0.5% respectively.

This multidisciplinary approach provides satisfactory post-operative analgesia allowing early safe ambulation and discharge from hospital. Anticipated problems did not arise, with early discharge not being detrimental to flexion achieved at six weeks and infection rates not increasing with the use of intra-articular catheters.

A consensus regarding management of calcaneal fractures eludes orthopaedic surgeons. While operative treatment has gained more acceptance, surgical morbidity remains high. We undertook a retrospective review of early complications and radiological outcomes following internal fixation of calcaneal fractures in our unit over 15 months.

A consecutive series of 33 patients who underwent fixation of the Calcaneus was selected. Patient records, X-rays and scans were reviewed.

37 fractures in 33 patients underwent fixation. 81 % were male. Mean age at surgery was 37yrs (19 -59yrs). 35 % were operated within 1 wk of injury and 13% after 2 wks of injury. 43% were Sanders' type III, 18% type II and 13% tuberosity avulsion fractures. 63% had a reversed/zero Bohler's angle. Mean post-op Bohler's angle was 32 degrees. Overall complication rate was 32%, with a combined deep infection rate of 8%. All patients with infection were male, and 70% were smokers. 86% were above 40yrs of age (mean 47yrs). The deep infection rate for intra-articular fractures was 3% and for tuberosity avulsion fractures 40%. Majority of patients with wound complications had been operated within 7 days of injury.

Males over 40yrs and smokers seem to be at most risk of wound complications. Time to surgery/delay in surgery up to 2 wks did not seem to have any adverse consequences. Complications increase with fracture complexity and avulsion fractures have highest risk of wound breakdown. Near anatomical restoration of the articular surface is possible in most.

Teaching programmes for Orthopaedic Trainees take place in every region of the UK. The aim of this study was to survey Trainees from each region to compare levels of satisfaction.

A questionnaire was sent out electronically in June 2009, and was open to collect responses until December 2009. It was sent to all British Orthopaedic Trainees Association (BOTA) members to assess satisfaction and to ask for their comments and suggestions.

There were 449 responses, out of a potential 1200 (37%) from across all 26 regions. Overall 71% were satisfied or highly satisfied with the organisation of their programme, and 70% were satisfied or highly satisfied with the content. However, on analysis of the data looking at individual regions, there were marked differences in satisfaction levels. In three regions over 50% of Trainees were very satisfied with their teaching, in contrast to one region where 67% were unsatisfied or very unsatisfied.

199 Trainees (44%) made comments, which fell into themes of ‘how’ the teaching is organised, ‘what’ is taught, and by ‘whom’. Topics frequently mentioned were relevance to the FRCS exam, importance of clinical case presentations and involvement of Consultants.

Teaching Programmes are run differently in each region and there are contrasting levels of satisfaction amongst Trainees. The feedback from the Trainees in each region was fed back to the relevant Training Programme Director, and suggestions were made on how to improve these Teaching Programmes for the future.

Talar neck fractures are associated with high complication rates with significant associated morbidity. Adequate exposure and stable internal fixation remains challenging. We investigated the anterior extensile approach for exposure of these fractures and their fixation by screws introduced through the talo-navicular articulation. We also compared the quality and quantity of exposure of the talar neck obtained by this approach, with the classically described medial/lateral approaches.

An anterior approach to the talus between the tibialis anterior and the extensor hallucis tendons protecting both the superficial and deep peroneal nerves was performed on 5 fresh frozen cadaveric ankles . The surface area of talar neck accessible was measured using an Immersion Digital Microscribe and analysed with Rhinoceros 3D graphics package. Standard antero-medial and antero –lateral approaches were also carried out on the same ankles, and similar measurements taken. Seven talar neck fractures underwent operative fixation using the anterior approach with parallel cannulated screws inserted through the talo-navicular joint.

3D mapping demonstrated that the talar surface area visible by the anterior approach (mean 1200sqmm) is consistently superior to that visible by either the medial or lateral approaches in isolation or in combination. Medial malleolar osteotomy does not offer any additional visualisation of the talar neck. 3D reconstruction of the area visualised by the three approaches confirms that the anterior approach provides superior access to the entirety of the talar neck. 5 male and 2 female patients were reviewed. All had anatomical articular restoration, and no wound problems. None developed non union or AVN.

The anterior extensile approach offers superior visualisation of the talar neck in comparison to other approaches for anatomical articular restoration. We argue that this approach is safe, adequate and causes less vascular disruption.

The extended lateral approach offers a safe surgical approach in the fixation of calcaneal fractures. Lateral plating of the calcaneum could put structures on the medial side at risk. The aim was to identify structures at risk on the medial side of the calcaneum from wires, drills or screws passed from lateral to medial.

Ten embalmed cadaveric feet were dissected. A standard extended lateral approach was performed. The DePuy perimeter plate was first applied and 2mm K-wires were drilled through each of the holes. The medial side was now examined to determine the structures at risk through each hole. The process was repeated with the Stryker plate. The calcaneum was divided into 6 zones, by two vertical lines, from the margins of the posterior facet and a transverse line along the axis of the bone through the highest point of the peroneal tubercle.

The DePuy and the Stryker plates have 12 screw positions, 5 of which are common. With both systems, screw positions in zone 1 risk injury to the medial plantar nerve and zone 3 the lateral plantar nerve. A screw through zone 2 compromises the medial plantar in both. Screws through zone 4 risk the lateral plantar nerve with the DePuy plate. Screws through zone 5 of the DePuy plate risk the medial calcaneal nerve. Zone 5 of the Stryker plate and Zone 6 of both are safe.

There is significant risk to medial structures from laterally placed wires, drills or screws. Subtalar screws have the highest risk and have to be carefully measured and placed. The Stryker plating system is relatively safer than the DePuy perimeter plate with three safe zones out of six.

Pelvic fractures in children are uncommon. Despite their potentially serious nature, there is little information in the literature regarding their epidemiology.

We performed a retrospective review of case notes and radiographs, if available, of all patients admitted with bony pelvic injuries to our unit over a 28 year period (1980-2008). Sixty-four children with pelvic fractures were identified (median age 8 years, 75% male). 58% were pedestrians involved in road traffic accidents (RTA), 19% were caused by a fall from a height and 8% were crush injuries. There was seasonal variability, with a 50% increase in pelvic injuries in Autumn and 33% decrease in Winter - compared to Spring/Summer months (in contrast to the peak in overall fracture incidence in the Summer). Median length of stay was 8 days (range 1-180), 11% requiring direct admission to ITU and 5% to HDU. There is a high incidence of associated major injuries - for Stable fractures 52%, Partially stable fractures 60% and Unstable fractures 100%. Five required surgery to the bony pelvis, and 6 for-non pelvic orthopaedic injury. 3 required operative intervention from other specialities, 1 urological repair, 1 laparotomy and 1 lobectomy.

Pelvic fractures in children are potentially serious injuries with a high proportion of major associated injuries. Multiple injuries were sustained in 55% of pedestrian/RTA accidents, 100% of crush injuries and 58% of falls from a height. In this series, 52% of “Stable” pelvic injuries were associated with other major injuries. All paediatric pelvic injuries, even stable pubic rami fractures should be approached with caution, full ATLS assessment and close monitoring.

Delay to theatre after hip fracture is common in order to medically optimise the patient prior to surgery. The association between delay to surgery and mortality after hip fracture remains a contentious issue.

We aimed to investigate how medical postponement, time to surgery and correction of medical abnormalities prior to surgery affect peri-operative mortality after hip fracture.

From February to December 2007 prospective data was collected from all acute trauma units in Scotland relating to hip fracture patients' fitness for theatre, reasons for postponement of surgery and subsequent plans of action. The data-set recorded whether medical abnormalities were identified following criteria reported by McLaughlin et al. Survival at 30-days post-operation was used as primary outcome measure. Multivariable logistic regression models were used to control for differences in case-mix between patients.

Data were available for 4284 patients. Patients postponed for medical reasons were less likely to survive to 30 days compared to patients who were not postponed (87% (122/947) versus 93% (3098/3337)). Survival also decreased as time to theatre increased - 92% of patients operated on during the same/next day vs. 89% of those operated on admission day four. However, after controlling for differences in case-mix variables and co-morbidities, neither variable significantly affected survival. We then analysed whether delaying surgery to resolve medical problems improved survival. Adjusted survival was not significantly different between those patients who had their medical problem resolved prior to surgery compared to those patients who were not postponed. Individuals who were postponed but did not have their clinical abnormality resolved prior to surgery had significantly lower adjusted 30 day survival.

The possible benefits of postponement need to be weighed against prolonged discomfort and the possibility of developing other complications. Postponing patients who cannot be medically improved should be avoided.

Massive, irreparable rotator cuff tears occur in about 15% of patients with ruptures of the rotator cuff tendons. There is no consistently agreed management for irreparable rotator cuff tears, however, latissimus dorsi tendon transfer is a recognised technique. We aimed to review the functional outcome of patients undergoing this operation at a single tertiary referral centre.

Fourteen latissimus dorsi transfer procedures in thirteen patients from May 2007 to May 2008 were retrospectively reviewed. The mean age of patients undergoing the procedure was fifty nine years. All patients were confirmed to have massive, irreparable (>5cm) rotator cuff tears as determined by MRI or ultrasound. Modified Constant scores (assessing shoulder pain, functional activity and movement) determined pre-operatively and post latissmus dorsi transfer were compared. The mean duration of follow up was 12 months.

The mean Modified Constant Score (maximum = 75) improved from 23 points pre-operaively to 52 points post latissimus dorsi transfer (p < 0.05). All patients had improvement in shoulder pain following the operation. There was a trend for younger patients to have greater improvement in functional activity and shoulder movement.

From our series, latissimus dorsi transfer is effective at improving functional outcomes in patients with massive, irreparable rotator cuff tears, especially in younger age groups.

The aim of management of an adult distal humeral fracture is to restore mobility, stability and pain-free elbow function. Good results are usually achieved in the majority of fractures treated with ORIF, but the management of comminuted fractures in elderly, frail patients with osteoporotic bone remains controversial. The literature focuses on elbow replacement if stable internal fixation cannot be achieved, with “bag-of-bones” management now rarely discussed eg. key-note paper - 10 successful cases reported by Brown RF & Morgan RG in 1971 (JBJS 53-B(3):425-428). We present the experience in two units in which conservative management has been actively adopted in selected cases by consultants with a subspecialty interest in the elbow.

All patients over the age of 60 with distal humeral fractures (2007 – 2009) who had been treated conservatively were reviewed clinically and radiologically. Duration of follow-up and outcome, including the Oxford and quick DASH scores, were recorded, with the fractures classified using the AO system.

There were 25 patients, 19 female and 6 male. 19/25 patients have been successfully treated conservatively with a mean Range Of Movement: Extension/Flexion: 45/125, Pronation/Supination 74/70. Only 5 underwent subsequent total elbow replacement and one delayed ORIF. There is a significant complication rate following surgical treatment with ORIF or elbow replacement in elderly, frail patients, including infection, painful non-union and/or stiffness. We believe that there is a role for initial conservative treatment in selected patients with low, displaced, comminuted humeral fractures in osteoporotic bone. Initial early mobilisation as pain allows can give good functional results without the risks of operation. It does not preclude future surgery if conservative treatment fails, but this is not required in the majority of cases.

Total knee arthroplasty (TKA) is a common orthopaedic procedure. Traditionally the surgeon, based on experience, releases the medial structures in knees with varus deformity and lateral structures in knees with valgus deformity until subjectively they feel that they have achieved the intended alignment. The hypothesis for this study was that deformed knees do not routinely require releases to achieve an aligned lower limb in TKA.

A single surgeon consecutive cohort of 74 patients undergoing computer navigated TKA was examined. The mechanical axes were taken as the references for distal femoral and proximal tibial cuts. The trans-epicondylar axis was taken as the reference for frontal femoral and posterior condylar cuts. A soft tissue release was undertaken after the bony cuts had been made if the mechanical femoro-tibial (MFT) angle in extension did not come to within 2° of neutral as shown by computer readings. The post-operative alignment was recorded on the navigation system and also analysed with hip-knee-ankle (HKA) radiographs.

The range of pre-operative deformities on HKA radiographs was 15° varus to 27° valgus with a mean of 5° varus (SD 7.4°). Only two patients required a medial release. None of the patients required a lateral release. The post implant navigation value was within 2° of neutral in all cases. Post-operative HKA radiographs was available for 71 patients. The mean MFT angle from radiographs was 0.1° valgus (SD 2.1°). The range was from 6° varus to 7° valgus but only six patients (8.5%) were outside the ±3° range. The kinematic analysis also showed it to be within 2 degrees of neutral throughout the flexion making sure it is well balanced in 88% cases.

This series has shown that over 90% of patients had limbs aligned appropriately without the need for routine soft tissue releases. The use of computer assisted bone cuts leads to a low level of collateral release in TKA.

The routine use of a fixed distal femoral resection angle in total knee arthroplasty (TKA) assumes little or no variation in the angle between the anatomical and mechanical femoral axes (FMA angle) in different patients. The aims of this study were threefold, firstly to investigate the distribution of FMA angle in TKA patients, secondly to identify any correlation between the FMA angle and the pre-operative coronal mechanical femoro-tibial (MFT) angle and in addition to assess post-operative MFT angle with fixed or variable distal femoral resection angles.

277 primary TKAs were performed using either fixed or variable distal femoral resection angles (174 and 103 TKAs respectively), with intramedullary femoral and extramedullary tibial jigs. The variable distal femoral resection angles were equal to the FMA angle measured on pre-operative Hip-Knee-Ankle (HKA) digital radiographs for each patient. Outcomes were assessed by measuring the FMA angle and the pre- and post-operative MFT angles on HKA radiographs.

The FMA angle ranged from 2° to 9° (mean 5.9°). Both cohorts showed a correlation between FMA and pre-operative MFT angles (fixed: r = -0.499, variable: r = -0.346) with valgus knees having lower FMA angles. Post-operative coronal alignment within ±5° increased from 86% in the fixed angle group to 96% when using a variable angle, p = 0.025. For post-operative limb alignment within ±3°, accuracy improved from 67% (fixed) to 85% (variable), p = 0.002.

These results show that the use of a fixed distal femoral resection angle is a source of error regarding post-operative coronal limb malalignment. The correlation between the FMA angle and pre-operative varus-valgus alignment supports the rational of recommending the adjustment of the resection angle according to the pre-operative deformity (3°-5° for valgus, 6°-8° for varus) in cases where HKA radiographs are not available for pre-operative planning.

Anterior knee pain following primary total knee replacement (TKR) is a common problem with average reported rates in the literature of approximately 10%. Symptoms are frequently attributed to the patellofemoral joint, and the treatment of the patella during total knee replacement is controversial.

There is no article in the literature that the authors know of that has specifically evaluated the effect of patella rim cautery on TKR outcome. This is a denervation technique that has historically been employed, with no evidence base. A prospective comparative cohort study was performed to compare the outcome scores of patients who underwent circumferential patella rim cautery, with those who did not.

Patients who had undergone a primary TKR were identified from the unit's arthroplasty database. Two cohorts, who were age and gender matched, were established. None of the patients had their patella resurfaced, but all had a patellaplasty. The Low Contact Stress TKR (Depuy International) was used in all cases.

The effect of circumferential patella rim cautery on the Oxford Knee Score (OKS) and the more anterior knee pain specific Patellar Score (PS) a minimum of 2 years post surgery was evaluated. Previous reports have suggested that a change of 5 points in the OKS represents a clinical difference. A sample size calculation based on an effect size of 5 points with 80% power and a p-value of 0.05 would require a minimum of 76 patients in each group.

There were 94 patients who had undergone patellaplasty only, and 98 patients who had supplementary circumferential patella rim cautery during their primary TKR. The mean OKS were 34.61 and 33.29 respectively (p=0.41), while the PS scores were 21.03 and 20.87 (p=0.87).

No statistically significant differences were noted between the groups for either outcome score. Patella rim cauterisation is unnecessary in primary TKR.

Few studies have investigated the direct effect of bacteria and their products on articular cartilage chondrocytes ex vivo. An ex vivo model that allows the analysis of chondrocytes in situ would therefore be an important and exciting area of future research. It was hypothesised that a bovine cartilage explant model of septic arthritis would be an ideal model for providing fundamental information on the basic cellular mechanisms of cartilage destruction and chondrocyte death induced by bacterial infection uncomplicated by the immune response.

A fresh metacarpophalangeal joint from an abattoir slaughtered 3-year-old cow was skinned, rinsed in water and opened under sterile conditions. The cartilage explants were harvested using surgical scalpels and placed into a total of three tissue culture bottles (2 explants per bottle) containing 10ml Dulbecco's Modified Eagle Medium (DMEM). 50ml of a knee aspirate from a patient with septic arthritis, containing Group B streptococci (GBS), was added to bottle 1, 50ml of a negative knee aspirate was added to bottle 2 and 50ml DMEM to bottle 3.

The explants were incubated at 37°C for 24 hours. They were then stained with the fluorescent probes Chloromethylfluorescein Di-acetate (CMFDA) and Propidium Iodide and analysed using a Confocal Scanning Laser Microscope. Cell counts to assess percentage cell death were performed using Velocity 4 software.

There was strikingly more cell death observed at 24 hours in the cartilage explant exposed to bacteria in comparison to the non-infected controls. The percentage chondrocyte death was 43% in the presence of GBS, 0.8% in the presence of the negative aspirate and 0.2% in the presence of the DMEM control.

Although this is a very preliminary pilot study, it demonstrates an extremely rapid effect on the cartilage. Future bovine explant studies of septic arthritis will therefore be feasible and achievable.

Bizarre parosteal osteochondromatous proliferation (BPOP) is a benign lesion of bone originally described by Nora et al in 1983. To date there are no UK-based case series in the literature. Here we present the Scottish Bone Tumour Registry (SBTR) experience of this rare lesion.

A retrospective analysis of SBTR records was performed. Histological specimens were re-examined by a consultant musculoskeletal oncology pathologist. Radiographs were re-reported by a consultant musculoskeletal radiologist. From 1983-2009, 13 cases were identified; 6 male, 7 female. Age ranged from 13-65. All patients presented with localised swelling. Pain was present in 5 and trauma in 2. 9 lesions affected the hand, 3 the foot, and 1 the tibial tuberosity. 12 lesions were excised and 1 curetted. There were 7 recurrences of which 6 were excised. 1 patients' recurrence was not treated. 1 lesion recurred a second time. This was excised. There were no metastases. Radiographs typically showed densely mineralised lesions contiguous with an uninvolved cortex. Cortical breakthrough was present in 1 case and scalloping in another. Histology characteristically showed hypercellular cartilage with pleomorphism and calcification/ossification without atypia; bone undergoing maturation; and a spindle-cell stroma.

SBTR records indicate that BPOP is a rare lesion with no sex predilection that affects patients over a wide age range. Minor antecedent trauma was present in only 2 cases. In agreement with Nora et al. we feel that trauma is unlikely to represent an aetiological factor. Recurrence was over 50% in this series. Although this is similar to that found in other reports, it may indicate that more extensive resection is required for this aggressive lesion. Finally, although radiological/histological findings are often bizarre there have been no reported metastases and so it is important that BPOP is not mistaken for, or treated as, a malignant process such as chondrosarcoma.

Pyomyositis is a primary pyogenic infection in skeletal muscle, often progressing to abscess formation. It is rare in temperate climates and generally deep-seated within the pelvis with non-specific clinical features, making diagnosis difficult. Magnetic Resonance Imaging (MRI) is highly sensitive for muscle inflammation and fluid collection and with its increasing availability is now the investigation of choice. Treatment of pyomyositis abscess has traditionally been with incision and drainage or guided aspiration followed by a prolonged course of antibiotics, although there are sporadic reports of cases treated successfully with antibiotics alone. From our 20 year database of over 16000 paediatric orthopaedic admissions we identified only 3 cases with MRI-confirmed pyomyositis abscess. These were all in boys (aged 2-12) and affected the gluteal, piriformis and adductor muscles. Despite the organisms not being identified, each patient was treated successfully with a short (4-7 day) course of intravenous antibiotics followed by 2-6 weeks of oral therapy. There were no recurrences or complications and all made a full recovery. We propose that uncomplicated pyomyositis abscess in children may usually be managed conservatively without the need for open or percutaneous drainage.

The role of perioperative antibiotic prophylaxis in sarcoma surgery is well established. There are no guidelines for their use in this context but there is pressure from microbiologists to comply with agreed prophylaxis for joint arthroplasty despite major differences between patient groups and risks of infection in sarcoma surgery.

Two simple surveys were conducted online, the first for bone sarcoma surgery, the second for soft tissue sarcomas. An email was sent to the major centres worldwide conducting such surgery with links to the online surveys to assess current practice regarding antibiotic prophylaxis and surgical drains. The survey was limited to 8 questions, the emphasis being a simple survey, but included questions on indications, choice, duration of therapy as well as use, size and duration of surgical drains.

We received 38 responses from 15 countries to the bone sarcoma survey and 33 responses from 12 countries to the soft tissue sarcoma survey. Current antibiotic prophylaxis regimens varied widely among surgeons, emphasising the controversy that exists regarding what constitutes best clinical practice.

Opinions regarding use of perioperative antibiotic prophylaxis in sarcoma surgery vary widely among orthopaedic surgeons worldwide, illustrating the controversy as to what constitutes best clinical practice. This survey suggests the need for a randomised clinical trial to aid in the development of guidelines in this area.

Osteoid osteoma is a classically described benign bone tumour. Traditionally, the surgical treatment of choice was excision, but this can have significant morbidity. In recent years, percutaneous Radiofrequency Ablation (RFA) has grown in popularity as an alternative treatment. This study reports the outcomes using this technique in our regional bone tumour unit. Between May 2003 and October 2007, 14 patients (female, n=4; male, n = 10) aged 15 - 32yrs (mean age, 20.4yrs) underwent CT guided radiofrequency ablation treatment. These patients had typical radiograph, CT, MRI or isotope bone scan features of osteoid osteoma and had significant pain symptoms. The protocol for ablation in our institute is heating the tip of the electrode to 90°C for 6 minutes. All patients were subsequently offered follow up in the out-patient clinic. Outcomes were taken from the Scottish Bone Tumour Registry database.

11 patients (78.6%) patients had complete resolution of symptoms after one RF treatment. 3 (21.4%) cases were unsuccessful but 1 of these was due to technical failure. All 3 of the above patients had complete relief of symptoms after one further RF treatment. 1 (7.1%) patient initially had complete relief of symptoms, but suffered a recurrence after 9 months. This patient also had a second curative treatment. Follow up ranged from 3 – 18 months (mean 10 months). Percutaneous RFA for osteoid osteoma is an attractive treatment due to its efficacy and low morbidity. Our results showed a primary success rate of 78%, a secondary success rate of 100% (after one additional procedure) and a recurrence rate of 7.1%. These are comparable to previous reported series.

We believe our results add to the growing literature supporting radiofrequency ablation as the treatment of choice for osteoid osteoma.

The role of digital radiography has evolved in recent years. In many hospitals, radiographs have been completely digitised and moved to Picture Archiving and Communications System (PACS). Pre-operative templating for arthroplasty has been a major problem as a result.

We investigate the accuracy of Orthoview™ software in templating for hip and knee arthroplasty. A retrospective review of 20 Stryker-Exter hip and 20 Biomet-Vanguard knee arthroplasties was conducted. Anonymised preoperative radiographs were reviewed by experienced orthopaedic surgeons. Templated component sizes were compared with actual implanted component sizes. All radiographs were digitised on Kodak Carestream PACS. Five surgeons were asked to separately review the radiographs to avoid intra-observer error.

In templating for hip arthroplasty, Orthoview™ was 80% accurate in predicting the femoral stem size within one size of the actual component used. It predicted the offset with 100% accuracy. In 90% of patients, the actual head implant was within one size of the templated head. The system was able to predict the acetabular component size in only 30%. In knee arthroplasty, Orthoview™ was 80% accurate within one size of the actual component used for the femur and 90% for the tibia.

Orthoview™ enables the flexibility of digitised films to be used for pre-operative templating. It is reasonably accurate in prediction of femoral sizing in both hip and knee arthroplasty and tibial size in knee arthroplasty. It is considerably less useful for acetabular sizing. Surgeons should keep this variability in mind until more accurate systems are available.

Details of orthopaedic implants in Scotland are recorded on a national database. The results are used by the Scottish Arthroplasty Project to record survival and complication rates for both knee and hip replacements.

The aim of our study was to assess the accuracy of recorded data for unicompartmental knee replacements in the West of Scotland. The national database was searched for all unicompartmental knee replacements carried out in the West of Scotland between March 2000 to October 2004. All patient data was then crosschecked with hospital theatre records and case notes for confirmation of accuracy.

A total of 88 cases were coded as unicompartment joint replacements in the study period. 63 cases were confirmed as being accurate (71.6%) and 6 as being inaccurate (6.8%). 19 patient details were not available for review either from notes or theatre records (21.6%). Of those coded inaccurately, five were total knee replacements, one cemented hip hemiarthroplasty and one shoulder replacement. One case of miscoding could be accounted for as an error in documentation while in six cases no cause could be identified. Of the 63 knees confirmed as unicompartmental, seven knees had been revised within 5 years, giving a 5 year survival rate of 87.7%.

The current system used by the Scottish Arthroplasty Project in Scotland has at least a 6.8% inaccuracy rate when recording unicompartmental knee replacements.

The medial patellofemoral ligament is commonly torn in first patella dislocation and according to the literature occurs most frequently at the femoral insertion. As research has demonstrated the MPFL to be a primary soft tissue stabilizer of the patella there has been increased interest in acute repair of the ligament with the aim of reducing the risk of recurrent dislocation. MRI is generally used pre op to identify the site of rupture allowing planning of the surgical repair, however there is currently no published data regarding the correlation between surgical and MRI findings to confirm the reliability of this modality of imaging.

A retrospective analysis of all patients who were referred for MRI following patella dislocation was performed. The surgical and MRI findings of those who subsequently underwent MPFL repair were compared to look for any correlation. 41 patients were first time patella dislocators and 38 had MPFL rupture diagnosed on MRI. Of these 19 progressed to surgical intervention and 17 had adequate data available from the notes and to permit comparison. All of the MPFLs ruptured at the patella insertion. In 76.5% (13/17) of patients the site of MPFL rupture was correctly identified on MRI scan. Of those that were discordant 3/4 had the site of rupture identified as the femoral insertion on MRI and on 1/4 the MRI suggested rupture at both the femoral and patella insertions.

MPFL rupture is common in first time dislocations occurring in 93%% of this cohort. There was a clear predominance patella insertion rupture in our study which contrasts with the majority of the published literature. By correlating surgical and radiological findings we can conclude that while MRI can accurately diagnose MPFL rupture at the patella insertion but it is less accurate in identifying rupture at the femoral insertion.

Lumbar Spinal Canal Stenosis is a common condition in the ageing population. In Spinal decompression surgery a balance needs to be struck between the need to decompress the neural elements in the spinal canal and the risk of worsening the segmental instability that often coexists in this condition. Traditionally decompression has been supplemented with rigid stabilization e.g. fusion, which is irreversible. Recently semi-rigid or ‘soft’ stabilization philosophies have evolved. The Wallis Device is a second generation interspinous distraction/stabilization implant designed to achieve ‘soft’ segmental stabilization. In addition to stabilising the decompressed segment, it also provides a ‘block’ to full segmental extension, helping to maintain spinal canal dimensions even in the erect position.

We followed up and assessed outcomes in 50 patients (25 spinal decompression + Wallis implant and 25 spinal decompression alone). The two arms of the study were matched for gender, age and level of lumbar decompression. A single surgeon was involved in each case and carried out a standard procedure of fenestration and medial facetectomy. Outcomes were assessed during clinical follow-up as well as by telephone, and included the VAS, the Oswestry Disability Index (ODI) and the EQ5D Health Domain.

Early results suggest decreased incidence of recurrent symptoms and global improvement in all parameters measured, in those subjects that underwent Spinal decompression with Wallis stabilisation.

In recent years the majority of X-ray departments have moved to a digital format of recording and archiving radiographs. These digital images (as with previous ‘films’) have a built in magnification factor (variable with each patient), which, may cause errors in templating for joint replacement surgery. Placing a marker of known size at the same level as the joint in question allows calculation of the magnification. This may help to restore hip offset in total hip replacement.

To establish the magnification factor for digital radiographs taken in our unit.

To assess the usefulness of marker images in accurate preoperative templating. Preoperative marker radiographs were identified retrospectively. The apparent size of the marker was measured on digital image. This value was used to calculate the magnification of the image. The scaled X-ray was up loaded to a digital templating software programme. This software uses a ‘scaling tool’ to calculate the magnification of the image. The hip joint templating tool was the used to calculate the offset of the proximal femur, this was performed with the calculated magnification and also an assumed magnification of 120%. The recommended offset of Exeter V40 stem was noted for both values.

Images were identified for 40 patients with markers. The average magnification was 122% for both PACS and Orthoview with a range 113% – 129% and a standard deviation of 4%. The median value for magnification was 120%. The average change in offset between calculated and estimated magnification was 1.275mm with a maximum change of 3mm. In two cases this difference resulted in a change in the recommended offset (5%).

The use of marker radiographs is widely described. In this small series the magnification is the same as previously reported in other studies. The difference in offset between calculated and estimated magnification was relatively small and caused a change in the recommended offset in only two patients. Variation in the use of the templating tool in our software can produce a much greater change in offset. Marker radiographs will only be useful as part of a standardised method of pre-operative templating.

Upper limb lacerations are a common injury. Traditionally, these have been assessed and treated in the Emergency Department (ED). This has become increasingly rare. A number of different reasons have been postulated. These include the increasingly junior status of those that work within the ED and the 4 hour target within the ED. After referral to the orthopaedic department, these patients are often assessed by increasingly junior staff. Thus, when these patients are assessed by a sufficiently qualified practitioner, there exists no option but to take them to theatre for repair of their injury. The aim of our study was identify the number of patients requiring surgery for hand and wrist lacerations and identify if these patients could be managed without the need for theatre.

We collected data in a prospective fashion from 1/9/9 to 3/11/9 at a large district general hospital. Over this period, 36 patients required surgery for their hand or wrist laceration. 27 were male and 9 were female. The average age was 34 years. The average length of procedure from was 21 minutes. 32% of patients were admitted overnight.

In two thirds of cases, the operating surgeon felt the procedure could have been performed in a suture room rather than in theatre. Among those patients who could've been operated on within a suture room, 21 would not have required an admission for any other reason.

It seems likely that the number of referrals from the ED regarding patients with upper limb lacerations is not likely to decrease. It is, therefore, important to ensure that orthopaedic departments develop new ways of working to try and ensure that patients are treated in a prompt fashion. We believe that the addition of a suture room to the orthopaedic trauma room remains one possible way of achieving this.

Regional anaesthetic for foot surgery has been discussed as a method of post operative analgesia. Ankle block as the sole anaesthetic for foot surgery has not been extensively reviewed in the literature.

We aimed to describe our experience of forefoot surgery under ankle block alone. 21 consecutive forefoot procedures (18 patients) were carried out under ankle block. The blocks were performed by the senior authors. A mixture of 10ml 2% Lidocaine with 10ml 0.5 % Bupivacaine was administered to the superficial peroneal, deep peroneal, sural and saphenous nerves. Ankle tourniquet was employed in all procedures. The patients were contacted post operatively and completed a standardised questionnaire including an incremented pain assessment ranging from 0-10 (0 no pain, 10 severe pain). 17 female and 1 male patients were contacted (age range 33-67y). Procedures included 13 first metatarsal osteotomies, 3 cheilectomies, 2 first MTP joint replacements, and 5 fusions. 14 patients requested a short acting sedative (midazolam). 5 patients (27 %) reported some discomfort during the block procedure (average pain score 1.2). No patients reported any pain during their operation(s). 4 patients (22%) required supplementation of the block. Average pain score at 6, 12, 24 and 48 hours following surgery were 0.66, 2.9, 2.4 and 1.3 respectively. All patients were discharged home and walking on the same day. None complained of nausea or required parenteral analgesia; there were no readmissions. Each patient confirmed they would have surgery under local block rather than general anaesthesia and would recommend this technique to family and friends.

Forefoot surgery under ankle block alone is safe and effective. Anaesthesia obtained permits the majority of forefoot procedures and provides lasting post-operative analgesia. Combined with intraoperative sedation, use of ankle tourniquet and same day discharge, it has very high patient acceptance and satisfaction.

The ‘cement reaction’ is a recognised cardio-respiratory response to methylmethacrylate bone cement, characterised by hypotension, reduced cardiac output, and on occasion fatal circulatory collapse. It is seen in 0.5-1% of cemented hip arthroplasties during the insertion and pressurisation of cement into the femur, and is believed to be secondary to marrow thromboembolism, the vasodilatory effect of methylmethacrylate, or a combination of the two. A number of steps, within the operating surgeon's control, can be undertaken to reduce the risk of the ‘cement reaction’ occurring.

An e-mail based questionnaire was sent to all trainees and consultants in the West of Scotland containing eight questions relating to cementing technique when performing hemiarthroplasty of the hip. The questions related to measures to reduce the potential for ‘cement reaction’, e.g.: whether or not they routinely use a cement restrictor.

Seventy-two complete replies were received. For five of the eight measures, the surgeons routinely employed the suggested practices. For the remaining three, the consensus opinion was contrary to the suggested practice for reduction of the risk of ‘cement reaction’. These were with respect to the surgical approach employed, whether or not to attempt to remove all cancellous bone from the proximal femur, and the use, or not, of a venting tube during cement insertion. In all three cases, the difference was statistically significant on chi-squared testing.

The cohort of surgeons questioned routinely employ more than half of the methods suggested to reduce the potential for ‘cement reaction’ in hemiarthroplasty of the hip. Further surveys of why they do, or do not, undertake certain practices during cementing would help improve awareness of ‘cement reaction’, and perhaps reduce the incidence of this potentially fatal phenomenon.

We aimed to determine the impact of anaesthetic techniques on post-operative analgesic utilisation in people undergoing total knee arthroplasty. We conducted a prospective study at a district general hospital to assess post-operative analgesia use via patient controlled analgesia (PCA) in patients undergoing a elective total knee arthroplasty. Subjects were divided into three groups; group 1 (general anaesthetic and local nerve block, n = 39), group 2 (spinal only, n= 39) and group 3 (general anaesthetic only, n = 38). The primary outcome measure was post-operative morphine consumption through a PCA within the first 24 hours. All subjects were followed up post-operatively by a dedicated pain control nurse. Data was analysed using SPSS version 17 for Windows (SPSS Inc, Chicago, IL, USA).

Results are reported as mean (SD) unless stated otherwise. No significant difference in the age or sex distribution between groups was demonstrable. There was no statistical difference in the average usage of morphine when comparing Group 1 compared to group 2, (77.57 (49.56) vs 65.80 (44.71), p=0.27), group 1 compared to group 3, (77.57 (49.56) vs 77.80 (45.52) p=0.98) and finally group 2 compared to group 3, (65.80 (44.71) vs 77.80 (45.52), p=0.25).

In this cohort of patients undergoing total knee arthroplasty the anaesthetic technique used does not appear to have an influence on the post-operative pain management.

Forearm fractures are a common paediatric injury. This study was aimed to describe the epidemiological of paediatric forearm fractures in the urban population of Glasgow.

We reviewed of all the forearm fracture treated by the orthopaedic service in Yorkhill Children's Hospital in 2008. Datum gathered from case notes and radiographs using the prospective orthopaedic database to identify patients with forearm fractures. The age, sex, side and type of fracture, the timing and mechanism of the injury and treatment were documented for the 436 fractures. Census data were used to derive absolute age-specific incidences.

Distinction was made between torus and other types of fractures. Torus fractures require no specific orthopaedic treatment and were segregated out. For the remaining 314 fractures, the age and sex distribution, seasonal variation of fractures and treatments for each type of fracture were examined. The incidence of forearm fractures in our population is 411 fractures per 100,000 population per year. An increased number of fractures occurred during the months of May and August. A fall from less than one metre was the commonest mechanism of injury, sporting injuries were the second commonest with football the most common sport associated.

This study identifies some features which are in good agreement with studies from elsewhere in Britain, such as incidence and seasonality. However, there are also interesting differences – such as the Glasgow peak incidence for forearm fractures being at age 8, with a marked decline by 12 years. Furthermore, our findings have been extended to consideration of type of intervention, and likelihood of successful treatment.

The Ionising Radiation (Medical Exposure) Regulations 2000 is concerned with: “the making of safety measures in regard to radioactive substances and the emission of ionising radiation”. Responsibility is placed upon the Practitioner, Operator, Referrer and employer. A clinical evaluation of the outcome of each medical exposure must be recorded.” In Ayr Hospital Radiologist report A&E radiographs but not those in fracture clinic unless a specific request was made. Therefore the Surgeon/Trainee must record their interpretation.

An audit was completed to review the rate of documentation of the interpretation of radiographs in the clinical records of consecutive patients attending fracture clinics Notes and radiographs were reviewed from 6 separate fracture clinics. 106 patients attended during the time period and were seen by 9 different surgeons; 5 consultants and 4 training grade surgeons. 46 out of 106 patients were x-rayed and interpretation recorded in 38 cases. No interpretation was found in 8 cases, giving an overall compliance of 82.6% with no difference between different grades of surgeon: Consultants 79.2% (19/24); Trainees 86.4% (19/22).

These finding were presented at a departmental audit meeting and the audit loop was then closed by a second period of audit with an improvement in the consultants compliance to 100% (22/22) p=0.05 and no significant change in the trainees performance 75% (18/24).

Advantages of arthroscopic surgery in orthopaedic practice are well documented. The use and scope of ankle arthroscopy has evolved in the last decade. Its role in both the evaluation and treatment of chronic ankle pain has become more important with identification of newer pathologies. We aimed to identify the indications and complications of ankle arthroscopy in chronic ankle pain and to correlate the arthroscopic findings with pre-operative MRI/CT.

A retrospective analysis of all procedures done in our unit from 2005-2009. Patient records, X- rays and scans were reviewed. 77 patients were included in the study (46 male/31 female). The commonest age group was the 4th decade. There was a male preponderance in the younger age group (<50y), and a female preponderance in the older age groups (>50y). The commonest indication was impingement syndrome (44%/mean age 38y), followed by osteochondral lesions of the talus (23%/mean age 36y) and Osteoarthritis (22%/mean age56y). Other pathology included synovitis, Rheumatoid Arthritis, instability, AVN and combined pathologies. Pre-op MRI scans correlated with arthroscopic findings in 59%. The pathology most missed by MRI was impingement. 1 patient developed wound infection and another iatrogenic tendon rupture. 78% reported improvement in their symptoms following the procedure.

Ankle arthroscopy is a safe and effective procedure. It is particularly useful in the diagnosis and treatment of impingement syndromes and osteochondral lesions. Although there are serious recognised complications, their incidence is low. Patients with chronic symptoms and normal MRI/CT may have treatable pathology on arthroscopy.

The assessment of knee laxity by application of varus and valgus stress is a subjective clinical manoeuvre often used for soft tissue balancing in arthroplasty or for diagnosis of collateral ligament injuries. Quantitative adjuncts such as stress radiographs have enabled a more objective measurement of angular deviation but may be limited by variations in examination technique. The aim of this study was to quantify clinical knee laxity assessment by measurement of applied forces and resultant angulations.

A novel system for measuring the manually-applied forces and moments was developed. Both hardware and software components underwent laboratory validation prior to volunteer testing. Two clinicians performed multiple blinded examinations on two volunteers and the corresponding angular deviations were measured using a validated non-invasive system with a repeatability of ±1° for coronal alignment. The distance between the kinematically-determined knee and ankle centres was used as the moment arm.

Comparison of single measurements of laxity showed a wide intra- and inter-observer variation (up to 3°). However, when the median value of repeated measurements was used there was good repeatability for both a single surgeon on different days and between the two clinicians with angular measurements agreeing within 1°. In spite of this agreement, the magnitudes of the tangential forces and moments applied varied between clinicians and did not correlate with the corresponding angular deviations.

It was not possible to standardise clinical examination using the current system. Orientation of the applied force with respect to the leg was not quantified and during force measurement it became apparent that the assumed tangential direction of application was not true. This may explain the lack of correlation between the force and angulation data. However, for quantitative measurement of coronal knee laxity using non-invasive laxity measurements, the use of a repeated measures protocol may be accurate enough for clinical application.

Locking plate fixation in proximal humeral fractures has demonstrated good results tempered by a significant rate of loss of fixation. Reported rates of failure are typically around 10% of cases but can be 20% or higher. In addition large series are often made up of a diverse patient population, so we have chosen to focus solely on patients confirmed to have significantly reduced BMD who can be considered a subset at high risk of fixation failure.

Twenty-three patients (5 male, 18 female) with a proximal humeral fracture treated by locking plate fixation were confirmed on DEXA scanning to be osteopaenic (17), osteoporotic (4) or severely osteoporotic (2). Patients early in the series were reviewed retrospectively and recalled for an updated assessment where appropriate, and the remainder were followed prospectively. The average age was 66 years (range 49 to 82). Follow up was for an average of nine months following surgery (range 2 and a half to 28 months). 17 patients underwent surgery for acute injuries and 6 for established surgical neck non-unions.

Seven injuries were 2-part fractures, 12 3-part, 3 were 4-part and one a 2-part surgical neck non-union.1 plate failed due to complete loss of fixation within 2 months in a patient with severe osteoporosis and was treated with removal of metalwork. This was the only injury that failed to unite. Avascular necrosis occurred in three patients with two revised to a hemiarthroplasty. 1 patient had ongoing pain and underwent removal of the plate.

Our series demonstrated that locking plate fixation of proximal humeral fractures is associated with a low rate of fixation failure and satisfactory outcomes in patients with significantly reduced bone mineral density.
