It is probable that both genetic and environmental
factors play some part in the aetiology of most cases of degenerative
hip disease. Geneticists have identified some single gene disorders
of the hip, but have had difficulty in identifying the genetics
of many of the common causes of degenerative hip disease. The heterogeneity
of the phenotypes studied is part of the problem. A detailed classification
of phenotypes is proposed. This study is based on careful documentation
of 2003 consecutive total hip replacements performed by a single
surgeon between 1972 and 2000. The concept that developmental problems
may initiate degenerative hip disease is supported. The influences
of gender, age and body mass index are outlined. Biomechanical explanations
for some of the radiological appearances encountered are suggested.
The body weight lever, which is larger than the abductor lever, causes
the abductor power to be more important than body weight. The possibility
that a deficiency in joint lubrication is a cause of degenerative
hip disease is discussed. Identifying the phenotypes may help geneticists
to identify genes responsible for degenerative hip disease, and
eventually lead to a definitive classification.
We performed a retrospective examination of the
anteroposterior pelvic CT scout views of 419 randomly selected patients between
April 2004 and August 2009 in order to determine the prevalence
of cam-type femoroacetabular deformity in the asymptomatic population.
The CT scans had all been undertaken for conditions unrelated to
disorders of the hip. The frequency of cam-type femoroacetabular
deformity was assessed by measuring the α-angle of each hip on the
anteroposterior images. The α-angles were classified according to
the Copenhagen Osteoarthritis Study. Among 215 male hips (108 patients)
the mean α-angle was 59.12° (37.75° to 103.50°). Of these, a total
of 30 hips (13.95%) were defined as pathological, 32 (14.88%) as
borderline and 153 (71.16%) as normal. Among 540 female hips (272
patients) the mean α-angle was 45.47° (34.75° to 87.00°), with 30
hips (5.56%) defined as pathological, 33 (6.11%) as borderline and
477 (88.33%) as normal. It appears that the cam-type femoroacetabular
deformity is not rare among the asymptomatic population. These anatomical
abnormalities, as determined by an increased α-angle, appear to
be twice as frequent in men as in women. Although an association
between osteoarthritis and femoroacetabular impingement is believed
to exist, a long-term epidemiological study is needed to determine
the natural history of these anatomical abnormalities.
Large femoral heads have become popular in total
hip replacement (THR) as a method of reducing the risk of dislocation.
However, if large heads are used in ceramic-on-ceramic THR, the
liner must be thinner, which may increase the risk of fracture.
To compare the rates of ceramic fracture and dislocation between
28 mm and 32 mm ceramic heads, 120 hips in 109 patients (51 men
and 58 women, mean age 49.2 years) were randomised to THR with either
a 28 mm or a 32 mm ceramic articulation. A total of 57/60 hips assigned
to the 28 mm group and 55/60 hips assigned to the 32 mm group were
followed for at least five years. No ceramic component fractures
occured in any patient in either group. There was one dislocation
in the 32 mm group and none in the 28 mm group (p = 0.464). No hip
had detectable wear, focal osteolysis or prosthetic loosening. In
our small study the 32 mm ceramic articulation appeared to be safe
in terms of ceramic liner fracture. Cite this article:
Hip arthrodesis remains a viable surgical technique
in well selected patients, typically the young manual labourer with
isolated unilateral hip disease. Despite this, its popularity with
patients and surgeons has decreased due to the evolution of hip
replacement, and is seldom chosen by young adult patients today.
The surgeon is more likely to encounter a patient who requests conversion
to total hip replacement (THR). The most common indications are
a painful pseudarthrosis, back pain, ipsilateral knee pain or contralateral
hip pain. Occasionally the patient will request conversion because
of difficulty with activities of daily living, body image and perceived
cosmesis. The technique of conversion and a discussion of the results

Peri-acetabular tumour resections and their subsequent
reconstruction are among the most challenging procedures in orthopaedic
oncology. Despite the fact that a number of different pelvic endoprostheses
have been introduced, rates of complication remain high and long-term
results are mostly lacking. In this retrospective study, we aimed to evaluate the outcome
of reconstructing a peri-acetabular defect with a pedestal cup endoprosthesis
after a type 2 or type 2/3 internal hemipelvectomy. A total of 19 patients (11M:8F) with a mean age of 48 years (14
to 72) were included, most of whom had been treated for a primary
bone tumour (n = 16) between 2003 and 2009. After a mean follow-up
of 39 months (28 days to 8.7 years) seven patients had died. After
a mean follow-up of 7.9 years (4.3 to 10.5), 12 patients were alive,
of whom 11 were disease-free. Complications occurred in 15 patients.
Three had recurrent dislocations and three experienced aseptic loosening.
There were no mechanical failures. Infection occurred in nine patients,
six of whom required removal of the prosthesis. Two patients underwent
hindquarter amputation for local recurrence. The implant survival rate at five years was 50% for all reasons,
and 61% for non-oncological reasons. The mean Musculoskeletal Tumor
Society score at final follow-up was 49% (13 to 87). Based on these poor results, we advise caution if using the pedestal
cup for reconstruction of a peri-acetabular tumour resection. Cite this article:
The technical advances in arthroscopic surgery
of the hip, including the improved ability to manage the capsule
and gain extensile exposure, have been paralleled by a growth in
the number of conditions that can be addressed. This expanding list
includes symptomatic labral tears, chondral lesions, injuries of
the ligamentum teres, femoroacetabular impingement (FAI), capsular
laxity and instability, and various extra-articular disorders, including snapping
hip syndromes. With a careful diagnostic evaluation and technical
execution of well-indicated procedures, arthroscopic surgery of
the hip can achieve successful clinical outcomes, with predictable
improvements in function and pre-injury levels of physical activity
for many patients. This paper reviews the current position in relation to the use
of arthroscopy in the treatment of disorders of the hip. Cite this article:
Restoration of leg length and offset is an important
goal in total hip replacement. This paper reports a calliper-based technique
to help achieve these goals by restoring the location of the centre
of the femoral head. This was validated first by using a co-ordinate
measuring machine to see how closely the calliper technique could
record and restore the centre of the femoral head when simulating
hip replacement on Sawbone femur, and secondly by using CT in patients
undergoing hip replacement. Results from the co-ordinate measuring machine showed that the
centre of the femoral head was predicted by the calliper to within
4.3 mm for offset (mean 1.6 (95% confidence interval (CI) 0.4 to
2.8)) and 2.4 mm for vertical height (mean -0.6 (95% CI -1.4 to
0.2)).
The CT scans showed that offset and vertical height were restored
to within 8 mm
(mean -1 (95% CI -2.1 to 0.6)) and -14 mm (mean 4 (95% CI 1.8 to
4.3)), respectively. Accurate assessment and restoration of the centre of the femoral
head is feasible with a calliper. It is quick, inexpensive, simple
to use and can be applied to any design of femoral component.
The piriformis muscle is an important landmark
in the surgical anatomy of the hip, particularly the posterior approach
for total hip replacement (THR). Standard orthopaedic teaching dictates
that the tendon must be cut in to allow adequate access to the superior
part of the acetabulum and the femoral medullary canal. However,
in our experience a routine THR can be performed through a posterior
approach without sacrificing this tendon. We dissected the proximal femora of 15 cadavers in order to clarify
the morphological anatomy of the piriformis tendon. We confirmed
that the tendon attaches on the crest of the greater trochanter,
in a position superior to the trochanteric fossa, away from the
entry point for broaching the intramedullary canal during THR. The
tendon attachment site encompassed the summit and medial aspect
of the greater trochanter as well as a variable attachment to the
fibrous capsule of the hip joint. In addition we dissected seven
cadavers resecting all posterior attachments except the piriformis
muscle and tendon in order to study their relations to the hip joint,
as the joint was flexed. At flexion of 90° the piriformis muscle
lay directly posterior to the hip joint. The piriform fossa is a term used by orthopaedic surgeons to
refer the trochanteric fossa and normally has no relation to the
attachment site of the piriformis tendon. In hip flexion the piriformis
lies directly behind the hip joint and might reasonably be considered
to contribute to the stability of the joint. We conclude that the anatomy of the piriformis muscle is often
inaccurately described in the current surgical literature and terms
are used and interchanged inappropriately. Cite this article:
Atypical cartilaginous tumours are usually treated
by curettage. The purpose of this study was to show that radiofrequency
ablation was an effective alternative treatment. We enrolled 20 patients (two male, 18 female, mean age 56 years
(36 to 72) in a proof-of-principle study. After inclusion, biopsy
and radiofrequency ablation were performed, followed three months
later by curettage and adjuvant phenolisation. The primary endpoint
was the proportional necrosis in the retrieved material. Secondary endpoints
were correlation with the findings on gadolinium enhanced MRI, functional
outcome and complications. Our results show that 95% to 100% necrosis was obtained in 14
of the 20 patients. MRI had a 91% sensitivity and 67% specificity
for detecting residual tumour after curettage. The mean functional
outcome (MSTS) score six weeks after radiofrequency ablation was
27.1 (23 to 30) compared with 18.1 (12 to 25) after curettage (p
<
0.001). No complications occurred after ablation, while two
patients developed a pathological fracture after curettage. We have shown that radiofrequency ablation is capable of completely
eradicating cartilaginous tumour cells in selective cases. MRI has
a 91% sensitivity for detecting any residual tumour. Radiofrequency
ablation can be performed on an outpatient basis allowing a rapid
return to normal activities. If it can be made more effective, it
has the potential to provide better local control, while improving

Osteoporosis is common and the health and financial
cost of fragility fractures is considerable. The burden of cardiovascular
disease has been reduced dramatically by identifying and targeting
those most at risk. A similar approach is potentially possible in
the context of fragility fractures. The World Health Organization
created and endorsed the use of FRAX, a fracture risk assessment
tool, which uses selected risk factors to calculate a quantitative,
patient-specific, ten-year risk of sustaining a fragility fracture.
Treatment can thus be based on this as well as on measured bone
mineral density. It may also be used to determine at-risk individuals,
who should undergo bone densitometry. FRAX has been incorporated
into the national osteoporosis guidelines of countries in the Americas,
Europe, the Far East and Australasia. The United Kingdom National
Institute for Health and Clinical Excellence also advocates its
use in their guidance on the assessment of the risk of fragility
fracture, and it may become an important tool to combat the health
challenges posed by fragility fractures.
The term developmental dysplasia of the hip (DDH)
describes a spectrum of disorders that results in abnormal development
of the hip joint. If not treated successfully in childhood, these
patients may go on to develop hip symptoms and/or secondary osteoarthritis
in adulthood. In this review we describe the altered anatomy encountered
in adults with DDH along with the management options, and the challenges
associated with hip arthroscopy, osteotomies and arthroplasty for
the treatment of DDH in young adults. Cite this article:
Over recent years hip arthroscopic surgery has
evolved into one of the most rapidly expanding fields in orthopaedic surgery.
Complications are largely transient and incidences between 0.5%
and 6.4% have been reported. However, major complications can and
do occur. This article analyses the reported complications and makes recommendations
based on the literature review and personal experience on how to
minimise them.
Second-generation metal-on-metal bearings were introduced as a response to the considerable incidence of wear-induced failures associated with conventional replacements, especially in young patients. We present the results at ten years of a consecutive series of patients treated using a metal-on-metal hip resurfacing. A distinct feature of the bearings used in our series was that they had been subjected to double-heat treatments during the post-casting phase of their manufacture. In the past these bearings had not been subjected to thermal treatments, making this a unique metal-on-metal bearing which had not been used before in clinical practice. We report the outcome of 184 consecutive hips (160 patients) treated using a hybrid-fixed metal-on-metal hip resurfacing during 1996. Patients were invited for a clinicoradiological follow-up at a minimum of ten years. The Oxford hip score and anteroposterior and lateral radiographs were obtained. The mean age at operation was 54 years (21 to 75). A series of 107 consecutive hips (99 patients) who received the same prosthesis, but subjected to a single thermal treatment after being cast, between March 1994 and December 1995, were used as a control group for comparison. In the 1994 to 1995 group seven patients (seven hips) died from unrelated causes and there were four revisions (4%) for osteolysis and aseptic loosening. In the 1996 group nine patients died at a mean of 6.9 years after operation because of unrelated causes. There were 30 revisions (16%) at a mean of 7.3 years (1.2 to 10.9), one for infection at 1.2 years and 29 for osteolysis and aseptic loosening. Furthermore, in the latter group there were radiological signs of failure in 27 (24%) of the 111 surviving hips. The magnitude of the problem of osteolysis and aseptic loosening in the 1996 cohort did not become obvious until five years after the operation. Our results indicate that double-heat treatments of metal-on-metal bearings can lead to an increased incidence of wear-induced osteolysis.
Pseudotumour is a rare but important complication of metal-on-metal hip resurfacing that occurs much more commonly in women than in men. We examined the relationship between head-neck ratio (HNR) and pseudotumour formation in 18 resurfaced hips (18 patients) revised for pseudotumour and 42 asymptomatic control resurfaced hips (42 patients). Patients in whom pseudotumour formation had occurred had higher pre-operative HNR than the control patients (mean 1.37 ( We suggest that reducing the size of the femoral head, made possible by a high pre-operative HNR, increases the risk of impingement and edge loading, and may contribute to high wear and pseudotumour formation. As the incidence of pseudotumour is low in men, it appears safe to perform resurfacing in men. However, this study suggests that it is also reasonable to resurface in women with a pre-operative HNR ≤ 1.3.
Aseptic loosening of the femoral component is
an important indication for revision surgery in unicompartmental knee
replacement (UKR). A new design of femoral component with an additional
peg was introduced for the cemented Oxford UKR to increase its stability.
The purpose of this study was to compare the primary stability of
the two designs of component. Medial Oxford UKR was performed in 12 pairs of human cadaver
knees. In each pair, one knee received the single peg and one received
the twin peg design. Three dimensional micromotion and subsidence
of the component in relation to the bone was measured under cyclical
loading at flexion of 40° and 70° using an optical measuring system.
Wilcoxon matched pairs signed-rank test was performed to detect
differences between the two groups. There was no significant difference in the relative micromotion
(p = 0.791 and 0.380, respectively) and subsidence (p = 0.301 and
0.176, respectively) of the component between the two groups at
both angles of flexion. Both designs of component offered good strength
of fixation in this cadaver study. Cite this article:
Large-head metal-on-metal total hip replacement has a failure rate of almost 8% at five years, three times the revision rate of conventional hip replacement. Unexplained pain remains a feature of this type of arthroplasty. All designs of the femoral component of large-head metal-on-metal total hip replacements share a unique characteristic: a subtended angle of 120° defining the proportion of a sphere that the head represents. Using MRI, we measured the contact area of the iliopsoas tendon on the femoral head in sagittal reconstruction of 20 hips of patients with symptomatic femoroacetabular impingement. We also measured the articular extent of the femoral head on 40 normal hips and ten with cam-type deformities. Finally, we performed virtual hip resurfacing on normal and cam-type hips, avoiding overhang of the metal rim inferomedially. The articular surface of the femoral head has a subtended angle of 120° anteriorly and posteriorly, but only 100° medially. Virtual surgery in a normally shaped femoral head showed a 20° skirt of metal protruding medially where iliopsoas articulates. The excessive extent of the large-diameter femoral components may cause iliopsoas impingement independently of the acetabular component. This may be the cause of postoperative pain with these implants.
To study the vascularity and bone metabolism of the femoral head/neck
following hip resurfacing arthroplasty, and to use these results
to compare the posterior and the trochanteric-flip approaches. In our previous work, we reported changes to intra-operative
blood flow during hip resurfacing arthroplasty comparing two surgical
approaches. In this study, we report the vascularity and the metabolic
bone function in the proximal femur in these same patients at one
year after the surgery. Vascularity and bone function was assessed
using scintigraphic techniques. Of the 13 patients who agreed to
take part, eight had their arthroplasty through a posterior approach
and five through a trochanteric-flip approach.Objectives
Methods
The β-angle is a radiological tool for measuring the distance between the pathological head-neck junction and the acetabular rim with the hip in 90° of flexion in patients with femoroacetabular impingement. Initially it was measured using an open-chamber MRI. We have developed a technique to measure this angle on plain radiographs. Correlation analysis was undertaken to determine the relationship between the range of movement and the β-angle in 50 patients with femoroacetabular impingement and 50 asymptomatic control subjects. Inter- and intra-observer reliability of the β-angle was also evaluated. Patients with femoroacetabular impingement had a significantly smaller (p <
0.001) mean β-angle (15.6°, 95% confidence interval (CI) 13.3 to 17.7) compared with the asymptomatic group (38.7°, 95% CI 36.5 to 41.0). Correlation between internal rotation and the β-angle was high in the impingement group and moderate in the asymptomatic group. The β-angle had excellent inter- and intra-observer reliability in both groups. Our findings suggest that the measurement of the β-angle on plain radiography may represent a valid, reproducible and cost-effective alternative to open MRI in the assessment of the pathological bony anatomy in patients with cam, pincer and mixed femoroacetabular impingement.
