Entrapment of the superficial peroneal nerve was treated in 24 legs of 21 patients by fasciotomy and neurolysis; 19 of the patients were reviewed after a mean period of 37 months. Nine were satisfied with the result, another six were improved but not satisfied because of residual limitation of athletic activity, three were unchanged and one was worse. Conduction velocity in the superficial peroneal nerve had increased after operation, but the change was not significant. In five patients the nerve had an anomalous course and in 11 there were fascial defects over the lateral compartment. Chronic lateral compartment syndrome is an unusual cause of nerve entrapment. Operative decompression produces cure or improvement in three-quarters of the cases, but is less effective in athletes

Background: The incidence of nerve injury following ankle arthroscopy has a documented rate of 1% to 24%1-15. The intermediate branch of the superficial peroneal nerve is at most risk with an antero-lateral portal incision 6, 9–12. The superficial peroneal nerve (SPN) is often marked as part of pre-operative planning,1 despite there being little evidence of the effectiveness of this simple measure in reducing nerve injury in ankle arthroscopies. Methods: We reviewed 100 consecutive cases who had an anterior ankle arthroscopy between February 2005 and April 2009. All arthroscopies were performed by a single surgeon (PFR) with pre-operative marking of the SPN. All patients were interviewed by telephone to find out if there had been any temporary or long-term neurological problems following the surgery. Any patients with neurological complications were reviewed in clinic. Patients’ notes were reviewed for any documented complications. Their level of satisfaction and improvement of symptoms were also assessed. Results: We were able to trace 98% of patients. The average follow up was 15.3 months (1 to 39 months). The only neurological deficit in this series was in one case (1%) who developed sensory loss in the distribution of the medial branch of the SPN. 61% of the cases were highly-satisfied/satisfied, 23% were moderately satisfied and 16% were not satisfied with the outcome of their surgery. The reason quoted by the 16% unsatisfied patients was failure to improve their symptoms to their expected level or their need for another operation (41% of the unsatisfied group). Conclusion: The incidence of nerve injury in our series was 1%. This is a dramatic improvement on the majority of published studies 1–15. We believe that marking the SPN prior to surgery is a simple and essential measure in reducing the neurological complications of ankle arthroscopy

Aims. This systematic review aimed to summarize the full range of complications reported following ankle arthroscopy and the frequency at which they occur. Methods. A computer-based search was performed in PubMed, Embase, Emcare, and ISI Web of Science. Two-stage title/abstract and full-text screening was performed independently by two reviewers. English-language original research studies reporting perioperative complications in a cohort of at least ten patients undergoing ankle arthroscopy were included. Complications were pooled across included studies in order to derive an overall complication rate. Quality assessment was performed using the Oxford Centre for Evidence-Based Medicine levels of evidence classification. Results. A total of 150 studies describing 7,942 cases of ankle arthroscopy in 7,777 patients were included. The overall pooled complication rate was 325/7,942 (4.09%). The most common complication was neurological injury, accounting for 180/325 (55.4%) of all complications. Of these, 59 (32.7%) affected the superficial peroneal nerve. Overall, 36/180 (20%) of all nerve injuries were permanent. The overall complication rate following anterior ankle arthroscopy was 205/4,709 (4.35%) compared to a rate of 35/528 (6.6%) following posterior arthroscopy. Neurological injury occurred in 52/1,998 (2.6%) of anterior cases using distraction, compared to 59/2,711 (2.2%) in cases with no distraction. The overall rate of major complications was 16/7,942 (0.2%), with the most common major complication – deep vein thrombosis – occurring in five cases. Conclusion. This comprehensive systematic review demonstrates that ankle arthroscopy is a safe procedure with a low overall complication rate. The majority of complications are minor, with potentially life-threatening complications reported in only 0.2% of patients. Cite this article: Bone Joint J 2023;105-B(3):239–246

Percutaneous plating of the distal tibia via a limited incision is an accepted technique of osteosynthesis for extra-articular and simple intra-articular distal tibia fractures. In this study we identify structures are risk during this approach. Method. Thirteen unpaired adult lower limbs were used for this study. Thirteen, 16-hole synthes®LCP anterolateral distal tibial plates were percutaneously inserted according to the manufacturer instructions and confirmed by xray. Dissection was performed around the plate to examine the relation of nerves and soft tissue. Results. The neurovascular bundle was under the plate in one case. Over the horizontal limb of the plate, typically the superficial peroneal nerve had a variable course over all four screw holes. The anterior tibial artery coursed over hole number 3 and the Extensor hallucis longistendon was positioned over hole 3 or 4. The Anterior tibialis tendon skirted hole 4 in 12 cases. Over the vertical limb of the plate, the neurovascular bundle coursed over holes, 5 to 7, the superficial peroneal nerve over holes 5 to 7. Discussion. Meticulous attention is required when placing an anterolateral distal tibia plate using a MIPO technique. We recommend a larger initial incision to avoid entanglement of the superficial peroneal nerve under the plate. Over the anterior aspect of the tibia, an open technique with adequate neurovascular structure and tendon protection is necessary due to the variability of structure coursing over the plate. A bridging technique for placement of proximal locking screw should be made through a mini open incision and this is safe to do so proximally over holes 12 to 16. Caution is advised during placement of screws percutaneously from holes 1 to 12 however the neurovascular bundle courses commonly over holes 5 to 7. These landmarks also apply to the use of shorter anterolateral distal tibial plates

We diagnosed entrapment of the superficial peroneal nerve in 17 patients (19 legs) with a mean age of 41 years. In all cases, plain radiographs of the leg, nerve-conduction studies of the superficial peroneal nerve and measurement of the intramuscular pressure at rest after exercise were normal. Diagnostic tests for nerve compression during rest after exercise produced pain and clinical signs in all. We performed decompression of the superficial peroneal tunnel in 14 patients and local fasciectomy in three. Fourteen patients (80%) were free from symptoms or satisfied with the result

We describe a medial midline portal between the tendons of extensor hallucis longus and tibialis anterior for arthroscopy of the ankle. We dissected 20 cadaver specimens to compare the risk of neurovascular injury using this approach with that of using standard arthroscopic portals. Compared with the anterocentral portal, the medial midline was a mean of 11.2 mm further from the nearest branch of the superficial peroneal nerve and 10.3 mm further from the dorsalis pedis artery. This portal allows good access to the joint surface and intra-articular structures and has a lower risk of injury to the dorsalis pedis artery, deep peroneal nerve or the medial branch of the superficial peroneal nerve

Aims: Peroneal tendons possess a vascular supply through a mesotendineal structure named vinculum; vincula are identifiable with tendoscopy and are supposed to play a role in tendon healing response, due to prominent vascularity; aim of the study is to verify the feasibility of tendoscopy in evaluating peroneal tendons, to clarify the histological structure of peroneal vincula and to investigate the presence of nervous tissue, so formulating a hypothesis regarding the functional role of vincula. Methods: cadaver study was performed on 8 fresh-frozen ankles; dissection were conducted to verify accessibility of endoscope, proximity with superficial peroneal nerve and presence of vincula; samples of vincula were obtained; light microscopy and immunohisto-chemistry (anti-human S100 antibody) were performed, describing structure of vincula and identifying peripheral nerve fibers. 5 peroneal vincula biopsies were analyzed from patients affected by ankle instability and undergoing tendoscopy for peroneal tenosynovitis. Results: peroneal tendons are accessible along the whole common tendon sheath and a discrete distance between the endoscope and the superficial peroneal nerve is present in all specimens; a membranous mesotendineal structure was found in all specimens between both tendons and tendon sheath; macroscopic inspection revealed the presence of a vessel network, arising from the sheath toward the tendon; light microscopy of cadaver samples confirmed the presence of multiple vessel branches crossing the entire structure of the vinculum and identified nervous structures close to the vessels, resembling a neurovascular bundle; immunohistochemical analysis revealed nerve fibers in each specimen;tendoscopy in patients affected by ankle instability showed lesions of the vincula and histology from intraoperative biopsies showed presence of nerve fibers. Conclusions: tendoscopy is a useful tool to visualize peroneal tendons and to diagnose and treat different disorders; although literature provides no data about innervations of peroneal vincula, the study shows nervous fibers consistently present inside the intimate structure of vinculum; besides its function in repair and healing processes, this suggests a proprioceptive role of the vinculum in peroneal tendon physiology and lesion of vinculum could be an important element acting synergistically with joint capsule and surrounding tissues lesions and ultimately leading to clinical pictures of ankle instability

The aim was to report operative complications, radiographic and patient-reported outcomes following lateral tibial plateau fracture fixation augmented with calcium phosphate cement (CPC). From 2007–2018, 187 patients (median age 57yrs [range 22–88], 63% female [n=118/187]) with a Schatzker II/III fracture were retrospectively identified. There were 103 (55%) ORIF and 84 (45%) percutaneous fixation procedures. Complications and radiographic outcomes were determined from outpatient records and radiographs. Long-term follow-up was via telephone interview. At a median of 6 months (range 0.1–138) postoperatively, complications included superficial peroneal nerve injury (0.5%, n=1/187), infection (6.4%, n=12/187), prominent metalwork (10.2%, n=19/187) and post-traumatic osteoarthritis (PTOA; 5.3%, n=10/187). The median postoperative medial proximal tibial angle was 89o (range 82–107) and posterior proximal tibial angle 82o (range 45–95). Three patients (1.6%) underwent debridement for infection and 27 (14.4%) required metalwork removal. Seven patients (4.2%) underwent total knee replacement for PTOA. Sixty percent of available patients (n=97/163) completed telephone follow-up at a median of 6yrs (range 1–13). The median Oxford Knee Score was 42 (range 3–48), Knee injury and Osteoarthritis Outcome Score 88 (range 10–100), EuroQol 5-Dimension score 0.812 (range −0.349–1.000) and Visual Analogue Scale 75 (range 10–100). There were no significant differences between ORIF and percutaneous fixation in patient-reported outcome (all p>0.05). Fixation augmented with CPC is safe and effective for lateral tibial plateau fractures, with a low complication rate and good long-term knee function and health-related quality of life. Percutaneous fixation offers a viable alternative to ORIF with no detriment to patient-reported outcome

The saphenous nerve is classically described as innervating skin of the medial foot extending to the first MTP joint and thus is at risk in surgery to the medial ankle and forefoot. However, it has previously been demonstrated by the senior author that the dorsomedial branch of the superficial peroneal nerve consistently supplies the dorsomedial forefoot, leading to debate as to whether the saphenous nerve should routinely be included in ankle blocks for forefoot surgery. We undertook a cadaveric study to assess the presence and variability of the saphenous nerve. 29 feet were dissected from a level 10 cm above the medial malleolus, and distally to the termination of the saphenous nerve. In 24 specimens (83%), a saphenous nerve was present at the ankle joint. In 5 specimens the nerve terminated at the level of the ankle joint, and in 19 specimens the nerve extended to supply the skin distal to the ankle. At the ankle, the mean distance of the nerve from the tibialis anterior tendon and saphenous vein was 14mm and 3mm respectively. The mean distance reached in the foot was 5.1cm. 28% of specimens had a saphenous nerve that reached the first metatarsal and no specimens had a nerve that reached the great toe. The current study shows that the course of the saphenous nerve is highly variable, and when present usually terminates within 5cm of the ankle. The saphenous nerve is at risk in anteromedial arthroscopy portal placement, and should be included in local anaesthetic ankle blocks in forefoot surgery, as a significant proportion of nerves supply the medial forefoot

Introduction. Extensor digitorum brevis (EDB) transfer is a useful method for treating chronic ankle instability in selected patients. It adds strength to the anterolateral capsule and provides proprioceptive feedback to functionally unstable ankles. Method. A single surgeon of case series of patients undergoing EDB transfer for chronic ankle instability following sporting injuries between January 2003 and July 2011 was reviewed. All patients underwent arthroscopic procedures in a day case setting. Outcomes were measured using return to sporting activity and the Karlsson functional scoring system. Results. 67 patients underwent unilateral EDB transfer over the 102 month period. 49 patients were male and all patients were aged less than 45. Minimum follow up was 6 months and all patients were discharged by 15 months (median follow up 9 months). Post operative assessment demonstrated normal range of ankle movements in all cases. At 6 months all patients had returned to sporting activity, achieving pre injury activity by 9 months. Karlsson scores were above 85.3 minor complications were seen - a superficial wound infection, limited paraesthesia of a branch of the superficial peroneal nerve and a case of complex regional pain syndrome which underwent full remission with early multimodal therapy. Conclusion. This study forms the largest published single surgeon series for EDB transfers. The data contributes to the body of evidence supporting EDB transfers as a safe and effective day case procedure for treatment of chronic ankle instability in selected patients

The ‘gold standard’ for treatment of chronic ankle instability is Brostrom-Gould procedure, which is an ‘open’ surgery. Most authors recommend this type of operation even after an arthroscopic inspection of the joint. The arthroscopic methods, including thermal shrinkage, propose in the past didn’t received a widely spread support. We present a method of arthroscopic lateral ligament ‘repair’ with an anchor placed in the fibula. We used this technique on 31 consecutive patients (28 were available for follow-up). 21 patients were victims of work or traffic accidents and 3 had sport-related lesions. The patients were evaluated after a 24,5 month follow-up. The average post-op AOFAS score was 85,3 and average satisfaction was 3,77 (1 to 5). We had two ‘relapses’ (another sprain without instability on the stress XR). On 3 patients we had some wound healing problems and we had 3 lesions of the superficial peroneal nerve, one of these is persistent. One patient developed a deep vein thrombosis post operatively. With this method, we achieved good results with relatively few complications, even more considering that most of the patients were work-compensation cases. It’s a simple procedure, less ‘aggressive’ then the traditional one’s, producing an anatomical and functional repair of the lateral ligament

We reviewed the outcome of Agility total ankle replacements carried out in our institution between 2002 and 2006. Follow-up consisted of clinical and radiological review pre-operatively, at 6 weeks, 6 and 12 months, and annually until 10 years post-op. Clinical review included the American Orthopaedic Foot and Ankle Score, satisfaction and pain scores. 30 arthroplasties were performed in 30 consecutive patients. Pre-operative diagnosis was rheumatoid arthritis (16), primary osteoarthritis (12) and post-traumatic osteoarthritis (2). After a mean follow up of 6.2 years (1.4–10.1), 4 patients had died, and 20 out of the remaining 24 were available for follow-up. Complications included lateral malleoli fracture (3), superficial peroneal nerve injury (2), one early death, unrelated to the surgical procedure, delayed syndesmotic union (1), non-union (6) and deep infection (2), of which one underwent removal of the implant; the other receives long-term oral antibiotics. AOFAS scores improved from mean 40.4 pre-op to 83.5 post-op (p<0.001). Radiological assessment revealed 25 (93%) patients had lucency in at least one zone in the AP radiograph. We found a relatively high level of re-surgery and complications following Agility total ankle replacement. A 7% revision rate is much higher than would be tolerated in knee or hip arthroplasty, but compares favourably to other studies of TAR. Despite radiological loosening, and the high rate of re-surgery and complications; patients are generally satisfied with the procedure, reporting lower levels of pain and improved function. Overall, we feel that the Agility ankle is an acceptable alternative to arthrodesis, however patients should be warned of the risk of re-surgery

The saphenous nerve is classically described as innervating skin of the medial foot to the first MTP joint and thus is at risk in surgery to the medial ankle and foot. However, it has previously been demonstrated that the dorsomedial branch of the superficial peroneal nerve consistently supplies the dorsomedial forefoot, and therefore previous descriptions of the saphenous nerve maybe erroneous. We undertook a cadaveric study to assess the presence and variability of this nerve. 21 cadaveric feet were dissected from a level 5 cm above the medial malleolus, and distally to the termination of the saphenous nerve. In 16 specimens (76%), a saphenous nerve was present, of which 14 were anterior to the saphenous vein. Two of 16 nerves terminated above the medial malleolus. Therefore, only 14 of 21 specimens (66%) had a saphenous nerve present at the level of the medial malleolus. In seven of these 14 specimens (50%), the nerve terminally branched before the level of the tip of the malleolus. The mean distance reached in the foot was 46mm. Only two nerves reached the forefoot, at 97mm and 110 mm from the ankle joint respectively. At the ankle, the mean distance of the nerve from the tibialis anterior tendon was 9mm, and the saphenous vein 1.2mm. Discussion. Our study shows that the course of the saphenous nerve is highly variable, and when present usually terminates within 40mm of the ankle. Only 10% reach the first MTP joint. These findings are inconsistent with standard surgical text descriptions. The saphenous nerve is at risk in distal tibial screw placement and arthroscopy portal placement, and should be included in local anaesthetic ankle blocks in forefoot surgery, as a small proportion of nerves supply sensation to the medial forefoot

Acute trauma and repetitive nicrotrauma connected with certain athletic activities are oftenmentioned when describing the etiology of nerve entrapment syndromes. According to the literature it is obvious that nerve entrapment syndromes in athletes are not as rare as they were once considered to be. Certain sports or physical activities have been mentioned that lead to specific nerve entrapment syndromes – for example, cyclist’s palsy and bowler’s thumb. Unlike nerve entrapment syndromes, vascular and neurovascular syndromes in athletes seem to be more common and have been described in greater detail, while nerve entrapment syndromes in athletes have been reported only recently. To support this contention, I present currently available information about nerve entrapment syndromes in athletes. For each syndrome possible cause of compression, clinical symptoms and signs, and the most effective treatment is presented. On the upper extremity are described: spinal accessory nerve, thoracic outlet syndrome, brachial plexus, long thoracic nerve, suprascapular nerve, axillary nerve, musculocutaneous nerve, lateral ante-brachial cutaneous nerve, radial nerve above the elbow, radial tunnel syndrome, Wartenberg’s disease, distal posterior interosseous nerve, ulnar nerve at the elbow and in Guyon’s canal, median nerve at the elbow and in carpal canal, anterior interosseous nerve and digital nerves. The syndromes described on the lower extremity are: groin pain, piriformis muscle syndrome, pudendus nerve, meralgia paresthetica, sural nerve, common peroneal nerve, superficial peroneal nerve, deep peroneal nerve, tarsal tunnel syndrome, the first branch of the lateral plantar nerve, medial plantar nerve (jogger’s foot) and interdigital neuromas (metatrsalgia. In conclusion I stress that nerve entrapment syndromes must be considered in the diferential diagnosis of pain in athletes

Objective: Review study of the long-term results of Mittelmeier’s high tibial osteotomy for the treatment of degenerative osteoarthritis of the knee. Patients – Mehod: 110 patients (93 women – 17 men) aged from 50 to 75 years (average 65 years) with knee osteoarthritis of stage II-IV at Ahlback’s grading system, were treated with high tibial osteotomy, in a 15 year period (1988–2002). An overall of 114 osteotomies were performed to the abovementioned patients. All the osteotomies were performed with the same surgical technique, the Mittelmeier’s biplane osteotomy with the use of a specially designed blade plate. 87 patients (73 women – 14 men) were followed-up for a mean time of 10 years (2–14 years). The parameters that evaluated were the knee function, the axial alignment, the complication rate and finally the time that the osteotomy was converted to total arthroplasty. Results: According to IOWA knee score the result was excellent in 64 patients and good in 15 patients, whereas 9 patients had a poor end result. 3 patients needed a total knee arthroplasty after a meantime of 8 – 10 years. In addition, 2 more patients fulfill the indications of a total arthroplasty. The main complications that occurred were: anterior tibia compartment syndrome in 1 patient, superficial infection in 2 patients, 1 delayed union and 1 mechanical failure of the osteosynthesis. Additionally, in 36 patients there were problems at the site of the fibular osteotomy (injury or entrapment of the superficial peroneal nerve in 12 patients, unremitting pain in 24 patients). Conlusions: The high tibial osteotomy is an effective though technically demanding method for the operative treatment of the osteoarthritis of the medial compartment of the knee joint
