Acute post-operative urinary retention (POUR) is a recognized complication following lower limb arthroplasty. Its occurrence may have patient and ultimately medico-legal implications. Identifying high-risk patients and the associated risk factors pre-operatively, is vital to tackle this issue and reduce its occurrence, which ultimately, may enhance the overall success of our operations. Our aim was to assess the incidence of POUR following elective lower limb arthroplasty and analyze the related factors that could potentially predict the likelihood of developing POUR in our patient cohort. A prospective audit of 158 patients was conducted in our department. POUR was defined as inability to pass urine voluntarily within the first 24 hours following elective lower limb arthroplasty leading to the insertion of a urinary catheter. Surgical-related factors including intra-operative fluid use, type of spinal anesthetic, duration of surgery, time from surgery till insertion of a urinary catheter as well as patient-related factors including medication, urological history and Body Mass index (BMI) was collected and analyzed. 21 (13.3%) patients developed post-operative urinary retention, 11 (52%) and 10 (48%) following knee and hip replacements respectively. Of which, 19 (90.5%) were male and 2 (9.5%) were female with an average age of 66 yrs. 13 (62%) had a previous urological history and 10 (48%) were on retention associated medication. Bupivacaine as a spinal anesthetic was associated with an increased risk of developing post-operative urinary retention. The average time till catheter insertion was 14 hrs. Only 2 (10%) had an unsuccessful TWOC on discharge. Bupivacaine as a spinal anesthetic and a previous urological history can be considered as risk factors for the development of POUR. Pre-operative urinary catheterization should be considered in this high-risk group of patients

Recent debate about changing population demographics and growing demands of younger patients has suggested a future explosion in the requirements for primary and revision lower limb arthroplasty (TKA/THA). This could represent a significant challenge for healthcare providers. This study aimed to predict the demands for lower limb arthroplasty in Scotland from 2010–2035. Population figures (2004–2010) and projected population data (five year increments) were obtained from the National Records of Scotland. The numbers of arthroplasties from 2004–2010 were provided by the Scottish Arthroplasty Project. Data were divided into three age groups (40–69, 60–79, 80+). The first model used mean incidence for each age group from 2006–2010 applied to the projected population figures. The second used linear regression to give predicted incidences 2015–2035 which were then applied to the projected population. The third-for revisions – used incidence per number of primary arthroplasties. For primary TKA model 1, comparing to 2010, showed demand increasing by 10% in 2020 and by 31% (to 8,650 procedures) in 2035. Model 2 gave increases of 60% and 161% respectively. An increase was found across all age groups with 60–79 more than doubling and 80+ increasing fourfold by 2035 (model 2). The revision TKA models predicted between 670 and 2,000 procedures by 2035. For primary THA models 1 and 2 showed demand increasing by 40% in 2020 and then by 60% and 110% (11,000 and 14,500 procedures) in 2035 respectively. All age groups had increasing demand with 60–79 doubling and 80+ tripling by 2035 (model 2). The revision THA models predicted between 1,300 and 2,100 procedures by 2035. These projections show large increases in the numbers of both primaries and revisions over the next two decades. They highlight that current resources may be insufficient or the selection criteria for surgery may need to be revisited

In recent years there has been growing interest in enhanced recovery regimes in lower limb arthroplasty due to potential clinical benefits of early mobilisation along with cost-savings. Following adoption of this regime in a district general hospital, it was observed that traditional dressings were a potential barrier to its success with ongoing wound problems in patients otherwise fit for discharge. The aim of this audit was to assess current wound care practice, implement a potentially improved regime and re-evaluate practice. A prospective clinical audit was performed over a three month period involving 100 patients undergoing hip or knee arthroplasty. Fifty patients with traditional dressings were evaluated prior to change in practice to a modern dressing (Aquacel™ Surgical). Fifty patients were then evaluated with the new dressing to complete the audit cycle. Clinical outcome measures included wear time, number of changes, blister rate and length of stay. Statistical comparisons were performed using Mann Whitney or Fisher's Exact test (statistical significance, p<0.05). Wear time for the traditional dressing (2 days) was significantly shorter than the modern dressing (7 days), p<0.001, and required more changes (0 vs. 3 days), p<0.001. 20% of patients developed blisters with the traditional dressing compared with 4% with the modern dressing (p=0.028). Length of stay was the same for the modern dressing (4 days) compared with the traditional dressing (4 days). However, in the modern group 75% of patients were discharged by day 4 whereas in the traditional group this took until day 6. This audit highlights the problems associated with traditional dressings with frequent early dressing changes, blistering and delayed discharge. These adverse outcomes can be minimised with a modern dressing specifically designed for the demands of lower limb arthroplasty. Units planning to implement enhanced recovery regimes should consider adopting this dressing to avoid compromising patient discharge

High-intensity narrow-spectrum (HINS) light is a novel violet-blue light inactivation technology which kills bacteria through a photodynamic process, and has been shown to have bactericidal activity against a wide range of species. Specimens from patients with infected hip and knee arthroplasties were collected over a one-year period (1 May 2009 to 30 April 2010). A range of these microbial isolates were tested for sensitivity to HINS-light. During testing, suspensions of the pathogens were exposed to increasing doses of HINS-light (of 123mW/cm2 irradiance). Non-light exposed control samples were also used. The samples were then plated onto agar plates and incubated at 37°C for 24 hours before enumeration. Complete inactivation (greater than 4-log10 reduction) was achieved for all of the isolates. The typical inactivation curve showed a slow initial reaction followed by a rapid period of inactivation. The doses of HINS-light required ranged between 118 and 2214 J/cm2. Gram-positive bacteria were generally found to be more susceptible than Gram-negative.

As HINS-light uses visible wavelengths, it can be safely used in the presence of patients and staff. This unique feature could lead to its possible use in the prevention of infection during surgery and post-operative dressing changes.

Introduction and Objective. Tranexamic acid (TXA) is used across surgical specialties to reduce perioperative bleeding. It has been shown to be effective in trauma, spinal surgery, and lower limb arthroplasty. The aim of this study is to investigate the clinical effectiveness of TXA in all types of shoulder surgery on bleeding and non-bleeding related outcomes. Materials and Methods. This study was registered prospectively on the PROSPERO database (ref: CRD42020185482). A systematic review and meta-analysis of randomised controlled trials (RCTs) investigating intra-operative use of TXA versus placebo in any type of surgery to the shoulder girdle. Electronic databases searched included MEDLINE, EMBASE, PsychINFO, and the Cochrane Library. Risk of bias within studies was assessed using the Cochrane risk of bias v2.0 tool and Jadad score. Certainty of findings were reported using the GRADE approach. The primary outcome was total blood loss. Secondary outcomes included patient reported outcome measures, adverse events, and rate of blood transfusion. Results. Eight RCTs were included in the systematic review and data from 7 of these studies pooled in the meta-analysis. A total of 708 patients were randomized across the studies (406 received TXA, 302 received placebo). Studies included patients undergoing anatomic or reverse total shoulder arthroplasty, open Latarjet surgery, and arthroscopic rotator cuff repair. Pooled analysis demonstrated significant reduction in perioperative bleeding with TXA compared to controls; estimated total blood loss (mean difference [MD], −209.66; 95% CI −389.11 to −30.21; p=0.02), and post-operative blood loss (via drain output) (MD, −84.8ml; 95% CI, −140.04 to −29.56; p=0.003). A mean difference in Visual Analogue Scale (VAS) of 2.93 was noted in favour of TXA (95% CI 0.2 to 5.66; p=0.04). Conclusions. Whilst noting some risk of bias within the studies, TXA was effective in reducing blood loss and pain in shoulder surgery. There may be a benefit of TXA use in both open and arthroscopic shoulder procedures. Larger, low risk of bias, RCTs for specific surgical shoulder procedures are required

Background-. Health care is best delivered face to face, doctor to patient. However, in some places like Scotland, patients can be in remote areas, far from the nearest health care provider. Medical video conferencing (VC) enables patients and doctors to meet for consultations from wherever they may be without the need for travel, and is already used widely in countries like Australia and Canada. Aim-. To do a pilot study of using the existing VC facility at our hospital for surgical pre-assessment of patients for elective foot/ankle and lower limb arthroplasty surgery. Methods- A prospective pilot study was performed at our hospital after approval from our ethics committee. Patient-records were vetted to include/exclude from the study and cases considered as “straightforward” were included. Two separate rooms with VC facility were set up in the orthopaedic outpatients, one with the patient and a trained physiotherapist, while the surgeon used the second room to discuss patient's complaints, do a physical examination, and discuss surgery where appropriate. Results-. 120 patients were included in the study out of which 82 (68 %) patients were seen exclusively through VC. Out of the 82, there were 42 foot/ankle cases and 40 cases for lower limb arthroplasty (TKR/THR). There were 40 cases where the VC consultation had to be converted into a face-to-face consultation mainly because the surgeon (31/40) was not entirely satisfied with the VC consultation (inadequate information in notes, anxious patient, etc). Conclusion-. We found that the VC consultation worked for a large majority of “straightforward” elective foot/ankle and lower limb arthroplasty conditions. We believe that there is certainly a role for VC consultation, especially for patients in rural/remote areas of Scotland, thus bringing down the cost and inconvenience of patient transport considerably

Background. While many studies have investigated long-term outcomes after lower limb arthroplasty, rather less is known about the trajectory of short-term outcomes in the first post-operative year. It is difficult for a surgeon to know when, in terms of disease severity, it is best to operate, or to make an accurate prediction of the patient pattern of post-operative recovery. We explored the trajectory of change in pain and function following primary hip and knee arthroplasty and the influence of pre-operative self-reported symptoms on post-operative improvements. Methods. A prospective UK cohort study of 164 patients undergoing primary hip (n=80) or knee (n=84) arthroplasty. WOMAC pain and function measures were collected pre-operatively and at 3 and 12 months post-operatively. Hip and knee arthroplasties were analysed separately, and patients were split into two groups: those with high or low symptoms pre-operatively. Multilevel regressions were used for each outcome (pain and function), and the trajectory of change (0–3 months and 3–12 months) charted. The study was approved by Southwest 4 Research Ethics Committee (09/H0102/72) and all patients provided informed, written consent. The authors have no competing interests to disclose. Results. Most improvement after hip arthroplasty occurred within the first 3 months post-operative. One year after surgery there was no difference in the outcomes between patients with high or low pain/function scores pre-operatively. Similarly, most improvement after knee arthroplasty occurred in the first 3 months. Improvement was greater for those patients with worse pre-operative functional disability and pain, although these patients never ‘caught-up’ with those with less severe pre-operative symptoms. Conclusions. Earlier intervention may be warranted for patients undergoing knee arthroplasty compared to hip arthroplasty to achieve the best post-operative patient-reported outcomes. Obtaining informed consent for an operation is a fundamental daily interaction between orthopaedic surgeon and patient. It is based on a patient's capacity to understand and retain information about the proposed procedure, the potential consequences of having it and the alternative options available. We used validated tests of memory on 59 patients undergoing lower limb arthroplasty to assess how well they learned and recalled information about their planned procedure. All patients showed an ability to learn new material, however, younger age and higher educational achievement correlated with better performance. These results have serious implications for orthopaedic surgeons discussing planned procedures. They identify groups of patients who may require enhanced methods of communicating the objectives, risks and alternatives to surgery. Further research is necessary to assess interventions to improve communication prior to surgery

Patient reported outcome measures (PROMs) are important for assessing the results of lower limb arthroplasty. Unrealistic or uneducated expectations may have a significant negative impact on PROMs even when surgery is technically successful. This study's aim was to quantify pre-operative expectations of Scottish patients undergoing total hip and knee replacement (THR/TKR). 100 THR and 100 TKR patients completed validated questionnaires (from the Hospital for Special Surgery) prior to their operation after receiving standard pre-operative information (booklet, DVD, consultations). Each patient rated expectations from very important to not having the expectation. A total score was calculated using a numerical scale for the grading of each expectation. Univariate regression analysis was used to investigate the relationship between demographics and expectation score. The THR cohort had mean age 66.2 (SD 10.5), 53% female, mean BMI 29.0 (SD 5.1) and mean Oxford score 44 (SD 7). The TKR cohort had mean age 67.6 (SD 8.5), 59% female, mean BMI 32.8 (SD 5.8) and mean Oxford score 44 (SD 8). 100% THR and 96% TKR patients had 10 or more expectations of their operation. All expected pain relief. Other improvements expected were: walking for 100% THA and 99% TKA patients; daily activities for 100% THAs and 96% TKAs; recreational activities for 96% THAs and 93% TKAs; sexual activity for 66% THAs and 59% TKAs; psychological well-being for 98% THAs and 91% TKAs. Regression analysis showed increasing age lowered expectations in both THR (p=0.025) and TKR (p=0.031) patients but that gender, BMI and Oxford score were not significantly related to expectations. This study highlights that patients expect far more than pain relief and improved post-operative mobility from their operation. It is important to discuss and manage these expectations with patients prior to surgery. By doing so, patient satisfaction and PROMs should further improve

Summary Statement. Service industry metrics (the net promoter score) are being introduced as a measure of UK healthcare satisfaction. Lower limb arthroplasty, as a ‘service’, scores comparably with the most successful commercial organisations. Background. Satisfaction with care is important to both the patient and the payer. The Net Promoter Score, widely used in the service industry, has been recently introduced to the UK National Health Service as an overarching metric of patient satisfaction and to monitor performance. This questionnaire asks ‘customers’ if they would recommend a service or products to others. Scores range from −100 (everyone is a detractor) to +100 (everyone is a promoter). In industry, a positive score is well regarded, with those over 50 regarded as excellent. Our aims were to assess net promoter scores for joint arthroplasty, to compare these scores with direct measures of patient satisfaction, and to evaluate which factors contributed to net promoter response. Methods. 6912 individuals undergoing primary lower limb joint replacement over a five year period (Jan 2007 – Dec 2011) took part in a prospective cohort study at a single NHS University hospital. Net promoter score, clinical outcomes as measured by PROMS (Oxford Hip or Knee Score and SF-12 score), multi-faceted patient satisfaction questionnaire, demographic data and length of hospital stay were recorded. Data was collected preoperatively and at 1 year post-surgery. Multivariate regression was performed to determine which factors could predict an outcome of ‘promoter’ and ‘detractor’ at 1 year post-surgery. Significance was accepted at p = 0.1 to accommodate the confounding effect of other variables. Results. Net promoter scores for knee and hip replacements were 49 and 71 respectively. Strong correlation was seen between overall satisfaction and whether the patient would recommend the operation to another (r = 0.637), though regression of these factors was modest (R. 2. = 0.406). Only 4 factors were relevant to the net promoter response: pain relief (OR 2.13, CI 1.83 – 2.49), meeting expectations (OR 2.57, CI 2.24 – 2.97), hospital experience (OR 2.33, CI 2.03 – 2.68) and arthroplasty type (OR 2.31, CI 1.68 – 3.17). These factors drove a model able to explain 95% of the variation in net promoter score. Conclusions. This is the first analysis of net promoter score for joint arthroplasty, and demonstrates values that compare favourably with the services provided by the most successful commercial organizations. The UK Department of Health describes this score as a measure of patient satisfaction. This is perhaps not completely accurate, as only a third of the variation in one response can be explained by the other, suggesting that although clearly related, these concepts are not the same. Pain relief, meeting of expectations of surgery, the hospital experience and whether the hip or knee joint is replaced are the only relevant factors in determining the net promoter response. Factors thought to influence clinical outcome such as depression, number of comorbidities, age and gender carry no influence with this metric
