The purpose of this study was to compare implant survivorship and functional outcomes in patients undergoing reverse total shoulder arthroplasty (RTSA) for acute proximal humeral fracture (PHF) with those undergoing elective RTSA in a population-based cohort study. Prospectively collected data from the New Zealand Joint Registry from 1999 to 2021 and identified 7,277 patients who underwent RTSA. Patients were categorized by pre-operative indication, including acute PHF (10.1%), rotator cuff arthropathy (RCA) (41.9%), osteoarthritis (OA) (32.2%), rheumatoid arthritis (RA) (5.2%) and old traumatic sequelae (4.9%). The PHF group was compared with elective indications based on patient, implant, and operative characteristics, as well as post-operative outcomes (Oxford Shoulder Score [OSS], and revision rate) at 6 months, 5 and 10 years after surgery. Survival and functional outcome analyses were adjusted by age, sex, ASA class and surgeon experience.Introduction
Methods
The number of Americans over the age of 80 is increasing at a faster rate than that of the 65–80 population. The cohort age 85–94 years had the fastest rate of growth from 2000–2010. The number of Americans older than 95 years grew at approximately 26% during the same period. This rapid growth has been associated with an increasing incidence of osteoarthritis of the hip and knee in this population. This surge in the growth rate of the elderly population has coincided with an increasing demand for primary and revision total joint arthroplasty. Surgeons need to be prepared to perform safely and appropriately these procedures in this rapidly growing segment of the population. Surgeons need to be aware of the 1) clinical outcomes that can be expected when total joint procedures are performed in this group of patients; 2) the morbidity and mortality associated with the performance of these procedures; and 3) the relative cost effectiveness of these interventions. Clinical outcomes of TJA in this population are generally good. Pain and satisfaction scores are similar to those of younger patients. Although pre-operative pain and functional impairment scores are higher pre-operatively in elderly patients, these improve significantly following TJA. However, functional outcome scores decline noticeably after 5 years, reflecting the impact of coexistent comorbidities. The continued need for assistive devices is greater in this age group than in younger total joint patients. The risk of falls, a particular issue of concern in this age group, is reduced after total hip and knee surgery. The rate of complications, including mortality, following TJA in this age group is greater than in the 65–79-year-old group. The use of hospitalists to co-manage peri-operative care is particularly important in this age group. The increased rate of complications is associated with longer lengths of stay. However, the length of stay for this age group after primary total joint replacement is decreasing significantly; reflecting the widespread streamlining of peri-operative care that is being incentivised and implemented nationwide. The use of extended care facilities is also greater in this age group. The performance of revision TJA in this age group is particularly challenging. The rates of revision in elderly patients are anticipated to rise significantly in coming years. Although revision TJA is associated with significant pain relief and patient satisfaction, it is accompanied by mortality and complication rates that are substantially greater than those in younger age groups.
For which patients is bone-defect-reconstruction with the Masquelet-technique suitable? Between 11/2011 and 1/2015 we treated 27 Patients (4 female/ 23 male) with bone-defects up to 150mm after septic complications with the Masquelet-technique. Reason of the bone defects were infected-non-unions of lower extremity, chronic osteomyelitis, infected knee-arthrodesis, chronic upper-ancle-empyema and infect-defect-non-union of the humerus. On average the patients were 47,5 (18–74) years old. The mean bone-defect-size was 62,6 mm (25–150). 26 of the 27 patients came from other hospitals, where they had up to 20 (mean 4,9) operations caused by the infection. The time before transfer to our hospital was on average 177days (6–720). 25 patients receaved flaps because of soft tissue-defects (7 free flaps, 18 local flaps). 13 patients suffered a polytrauma. In 5 cases the femur, in 3 cases a knee-arthrodesis, in 18 cases the tibia and in 1 case the humerus was affected by infection resulting in bone defects. Indication for the Masquelet-technique was low-/incompliance in 10 cases due to higher grade of traumatic brain injury and polytrauma and difficult soft-tissue conditions, in 6 times after problems with segment-transport and in 1 case as dead space management. Positiv microbial detection succeeded in 19 patients at the first operation although most of the patients underwent long term antibiotic therapy. Mainly we found problematic bacteria. At the time of defect reconstruction with spongious graft we found persistant bacteria in 4 cases. The first operation aimed treating the infection with radical sequestrectomy, removal of foreign bodies and filling the defect with an antibiotic loaded cementspacer as well as external fixation. 6–8 weeks later we removed the spacer and filled the defect with autologous bonegraft. In 2 cases we needed 2 bone grafts to fill the defect. In 9 cases we removed the fixateur and stabilized the defect with an internal anglestable plate. All patients were examined clinically and radiologically every 4–6 weeks in our outpatient-department for osteitis until full weight bearing and later every 3months In 22 of 27 cases the infection was clinically treated successfully. 5 patients are allowed for full weight bearing (all with secondary internal plates). No patient underwent amputation. There were 4 recurrences of infection, 9 instabilities needing internal stabilization and further bonegraft. For patients with low-/incompliance for various reasons and for those with difficult soft tissue conditions following flaps the Masquelet technique is a valuable alternative to the normal autologious spongegraft and to the segmenttransport. Internal fixation seems necessary.
For which patients is bone defect reconstruction with the Masquelet-technique after the treatment of osteomyelitis suitable and which results did we have. From 11/2011 to 4/2018 we treated 112 Patients (36f, 76m) with bone defects up 150mm after septic complications with the Masquelet-technique. We had infected-non-unions of upper and lower extremity, chronic osteomyelitis, infected knee-arthrodesis and knee- and ankle-joint-empyema. On average the patients were 52 (10–82) years old. The mean bone defect size was 48 mm (15–150). Most of our patients came from other hospitals, where they had up to 20 (mean 5.1) operations caused by the infection. Time before transfer in our hospital was on average 7,1 months (0,5–48). 77 patients received free (25) or local (52) flaps because of soft tissue-defects. 58 patients suffered a polytrauma. In 23 cases femur, in 4 cases a knee arthrodesis, in 68 cases tibia, in 1 case foot, 6 times ankle-joint arthrodesis, in 6 cases humerus, in 4 cases forearm were infected resulting in bone defects, In most cases the indication for the Masquelet-technique was low-/incompliance due to higher grade of brain injury and polytrauma followed by difficult soft tissue conditions and problems with segmenttransport. In 2/3 positive microbial detection succeeded at the first operation. Mainly we found difficult to treat bacteria. After treating the infection with radical sequestrectomy, removal of foreign bodies and filling the defect with antibiotic loaded cementspacer and external fixation we removed the spacer in common 6–8 weeks later and filled the defect with autologeous bone graft. Most of the patients needed an internal fixation after removing of the fixex. All patients were examined clinically and radiologically every 4–6 weeks in our outpatient department until full weight bearing, later every 3 Months.Aim
Methods
Metal-on-metal bearing surfaces were used frequently until recently because of the potential for decreased wear and the ability to use large femoral heads which can reduce instability. However, data reported in the Australian registry over the past 5 years demonstrated an increase in failure rates compared to metal-on-polyethylene bearings. In addition, adverse local tissue reactions (ALTR) associated with pseudotumors and destruction of the soft tissue around the joint have led to revision of these implants. Unfortunately, at the present time there is no optimal management strategy that has been delineated for metal-on-metal implants because of a lack of evidence. The biologic response to metal-on-metal implants is usually local but may be systemic. The management of these patients is complex because patients may have pseudotumors and/or elevated metal ion levels and be asymptomatic. In addition, there are a number of intrinsic causes (loosening, infection, iliopsoas tendinitis) and extrinsic causes (spinal disease, trochanteric bursitis) that can be a source of pain. There is no evidenced based approach to manage these patients and no single test should be used to determine treatment. A thorough clinical evaluation is essential and blood tests are necessary to rule out infection. Imaging studies including plain radiographs and a MARS MRI are needed to evaluate either the stability of the prosthesis and to assess for the presence of pseudotumors and soft tissue destruction around the implant. Patients with symptomatic hips and evidence of muscle involvement on MRI require revision. Finally, the clinician must always be aware of the potential for systemic effects which require early and aggressive intervention to limit the adverse response to the metal and the metal bearing.
Humeral resurfacing arthroplasty has been advocated as an alternative to stemmed humeral component designs given its ability to preserve proximal bone stock. Further, these implants have become more attractive given the possibility of stem-related complications including humeral fracture, stress shielding, and osteolysis; complications that may necessitate fixation, revision to long stem components, or reverse total shoulder arthroplasty. As more total shoulder arthroplasties are performed in younger patient populations, the likelihood of increased revision procedures is inevitable. Maintaining proximal bone stock in these cases with use of a resurfacing arthroplasty not only facilitates explant during revision arthroplasty, but preservation of proximal metaphyseal bone facilitates reimplantation of components. Clinical results of these resurfacing components have demonstrated favorable results similar to stemmed designs. Unfortunately, resurfacing arthroplasty may not be as ideal as was hoped with regard to recreating native humeral anatomy. Further, resurfacing arthroplasty may increase the risk of peri-prosthetic humeral fracture, and lack of a formal humeral head cut makes glenoid exposure more difficult, which may be associated with a higher degree of neurovascular injury. Stemless humeral components are designed for strong metaphyseal fixation and avoid the difficulty with glenoid exposure seen in resurfacing designs, as these components require a formal humeral head cut. Early clinical outcomes of a single stemless design demonstrated significant improvements in clinical outcome scores, without evidence of component migration, subsidence or loosening. The only mid-term clinical results of stemless design implants are seen with the Arthrex Eclipse system (Arthrex, Naples, FL). In a prospective study involving 78 patients at 5-year follow-up, significant improvements were observed in clinical outcome scores. While there was evidence of proximal stress shielding in an older population, this did not influence shoulder function. The overall revision rate was 9% at 5 years, with no component necessitating revision as a result of humeral component loosening. Resurfacing arthroplasty and stemless humeral components in total shoulder arthroplasty remain attractive options to preserve proximal metaphyseal bone stock, avoiding stem-related complications. Early and mid-term clinical outcomes are comparable to stemmed designs and demonstrate no evidence of humeral component loosening.
Osteoarthritis (OA) is one of the most common causes of knee pain in the aging population and presents with higher odds with increased BMI. Total knee arthroplasty (TKA) has become the standard of care for the treatment of OA. Over “719,000 TKA's were performed in 2010 in the USA alone, with dramatic economic burden- costing 16,000 USD per TKA” (CDC 2012). Over the past two decades, this cost was compounded by the unknown increasing rate of primary TKA and cannot be explained by the expanding population or worldwide obesity epidemic. These facts raise two key questions: are patients' quality of life expectations higher and driving the TKA rate up, or have surgeons changed their indications and started to operate on less disabled people? Our study aimed to determine the average functional profile for patients undergoing TKA using patient reported Outcome Measure (SF-36), to document if preoperative SF-36 scores have changed over the past two decades, and lastly to asses if patient pre-operative SF-36 scores are lower in in the USA vs the rest of the world. A literature search of Medline, Embase and Cochrane databases was performed extracting data from publishing year 1966 to 2016 with a search date of Dec 12, 2016. Two independent reviewers revised the abstracts and excluded articles with: no TKA, revision TKA, no pre-op SF-36, no SF-36 reported, incomplete scores to calculate SF-36, duplicates, review article, meta-analysis, letter to the editor, conference proceeding or abstract, disagreements were resolved with a third reviewer. All languages were included to maximize the catchment of data. All remaining articles were independently read and excluded if they did not provide data required for our study. Included articles were analyzed for data including: for year of patient enrollment, location (USA vs. non-USA), pre-operative SF-36 mental (MCS) and physical (PCS) component summary, level of evidence. Recorded data was compared post completion to assess inter-observer accuracy as per PRISMA guidelines for meta-analysis. After applying all the exclusion criteria on 923 selected abstracts, a total of 136 articles of which 30 were randomized control trials, were completely reviewed and included in our study. A total of 56,713 patients' physical component scores were analyzed and revealed an overall pre-operative SF-36 physical component score 31.93. When stratifying the data, it was revealed that patients operated in the USA had an average score of 32.3 whereas Non-US countries were 31.7, with no statistical significance. No statistical difference between SF36 scores was seen over time amongst studies of all nations. Based on the results of this study, we have shown that orthopaedic surgeons are performing TKA universally at the same pre-operative scores, independent of country of origin or year of surgery. The indications thus have remained consistent for two decades regardless of the advances in technology. Functional profiles of patients appear similar among US and Non-US countries. Further, we infer that based on pre-operative SF-36 PCS scores, the optimal time to undergo a TKA is when PCS is 31.9 +/−3.
Endoprosthetic replacement (EPR) is an available option for the management of massive bone loss around failed knee implants. The aim of this study was to determine the results of knee EPRs performed for non-tumour indications. Since 2007, 85 EPRs were performed for in a single tertiary centre by seven surgeons. Mean age at surgery was 73.5 years (range:35–95) with infection as the most common indication (49%), followed by aseptic loosening (18%), complex primary replacement (16%), fracture (15%) and mechanical failure (2%). Mean follow up was 4 years (range:1–9). Functional outcome was determined using the Oxford Knee Score (OKS). At follow-up, 21 patients were deceased and 2 lost to follow-up. Complication rate was 19%. Of the 7 infected cases, 6 were treated with DAIR (debridement, antibiotics, and implant retention) and one underwent above knee amputation. Four of the patients undergoing DAIR were cleared of infection and 2 are on long-term antibiotics. Accounting for implant revision, loss to follow-up and those on long-term antibiotics as failures, 5-year survival was 89% with an average OKS of 25 (SD=10). This mid-term study shows that distal femur EPR is a valuable option for the increasing burden of complex revision knee surgery

Indication for the Masquelet-technique was low-/incompliance in 35 cases due to higher grade of traumatic brain injury and polytrauma and difficult soft-tissue conditions, in 9 times problems with segment-transport and in 5 cases as dead space management. Positive microbial detection succeeded in 32 patients at the first operation. Mainly we found difficult to treat bacteria. After treating the infection with radical sequestrectomy, removal of foreign bodies and filling the defect with antibiotic loaded cement spacer and external fixation we removed the spacer6–8 weeks later and filled the defect with bone graft. In 23 cases we stabilized the defect then with an internal angle stable plate. All patients were examined clinically and radiologically every 4–6 weeks in our outpatient-department until full weight bearing, later every 3 months.Aim

Locked anterior shoulders (LAS) with static instability and anterior glenoid bone loss are challenging in the elderly population. Reverse shoulder arthroplasty (RSA) has been employed in treating these patients. No study has compared RSA for LAS to classically indicated RSA. A case-control study of patients treated with RSA for LAS with glenoid bone loss and static instability was performed using matched controls treated with primary RSA for classic indications. Twenty-four cases and 48 controls were evaluated. Average follow-up was 25.5 months and median age was 76. Motion, outcome assessments, and postoperative radiographs were compared.Background

The recent emergence of autologous blood concentrates, such as platelet rich plasma (PRP), as a treatment option for patients with orthopaedic injuries has led to an extensive debate about their clinical benefit. Our objective was to determine the effectiveness of autologous blood concentrates compared with control therapy in improving pain in patients with orthopaedic bone and soft tissue injuries. We conducted a systematic review of MEDLINE and EMBASE from 1996 and 1947, respectively, up to July 2010. Additional studies were identified by contacting experts, searching the bibliographies of the included studies as well as orthopaedic meeting archives. We included published and unpublished randomized controlled trials or prospective cohort studies that compared autologous blood concentrates with a control therapy in patients with an orthopaedic injury. Two reviewers, working in duplicate, abstracted data on study characteristics and protocol. Reviewers resolved disagreement by consensus.Purpose
Method
Most types of bone tumor surgery require intra-operative imaging or measurement to control margins and prevent unnecessary bone loss. Computer Assisted Surgery (CAS) has been used as a replacement of fluoroscopy or direct measurement tools in four specific types of oncological orthopaedic surgical approaches. There are intralesional treatments, image-based resections, image-based resections with image-based reconstructions and image-based resections with imageless tumor prostheses reconstruction. Since 2006 we have performed 130 oncological surgeries with CAS. Most cases were excochleations, 64, where CAS replaces fluoroscopy as an intra-operative imaging modality. Advantages over fluoroscopy are real time three dimensional feedback, high-res image and no use of ionizing radiation. It is especially useful in larger lesions or lesions located in the femoral head or pelvis. Currently a study is being performed on patient satisfaction, recurrence and complications. Another application where CAS has often been used is in resections and segmental resections (together 45). These can be preplanned before surgery, incorporating the margin required, and checked intra-operatively. Coloration of the tumor, critical structures is useful to avoid these. Sometimes it's possible with careful planning to spare structures that otherwise probably would not confidently have spared. With hemicortical resection (5) it's possible to use CAS to exactly copy the shape of the resected bone to an allograft. A Ct scan of one case shows an average gap between host and graft of 0.9 mm (range 0–5.4) along the 6 cm resection. Finally in 16 cases of imageless use in placement of tumor prostheses it feels greatly helpful in reconstructing the joint line, length and correct rotation. There were 8 failures in these 130 cases with the system or software. Setup time was measured in 47 cases and was on average 6:50 (range 2:26–14:27). Indication and performance of CAS in orthopedic oncology is an under researched aspect of CAS. In our opinion CAS shows great promise in the field of orthopedic oncology and is a valuable tool in the operating room.
Degenerate chondral surfaces can be assessed in many ways, but arthroscopy is often performed without proper categorisation, mapping, zoning or sizing of lesions. Progression of disease in un-resurfaced compartments is well-recognised to occur, but is only one of several failure mechanism in partial knee replacement. A validated ‘Functional Zone’ mapping method was used to document articular surface damage in 250 sequential cases of knee arthroscopy in patients over the age of 40. Size, shape, location and severity of each chondral lesion were noted using the Outerbridge classification. Analysis determined rates of involvement of particular compartments and assessed potential for partial replacement or local treatment and also to consider the risk of future progression. Radiographs including antero-posterior standing, postero-anterior flexion views (Rosenberg), lateral and skyline views were graded (Kellgren and Lawrence) and compared with the arthroscopic findings. Our results showed that out of the 210 knees with Grade 3 or greater damage 13.3% of knees showed ‘isolated’ medial disease of Outerbridge Grade 3 or worse. Isolated lateral disease was noted in 1.4%, patello-femoral disease in 24.3%, bi-compartmental (Medial/PFJ) disease in 30.9% with tibio-femoral and tri-compartmental disease seen in 15.2%. The combination of lateral and patello-femoral disease was seen in 14.8%. Provided that Grade 1 and 2 changes (which were found in other compartments in high percentages) were ignored and ACL status considered, this information seemed to indicate that at the time these procedures were performed, 13.3% of cases were suitable for a medial uni-compartmental device, with sub-analysis of lesion sizes indicating that 17 out of 28 cases (60.7%) were suitable for a localised resurfacing. Lateral uni-compartmental replacement seemed suitable for only 1.4%, patello-femoral replacement in 24.3%, bi-compartmental in 30.9% and total knee replacement in 30%. The mean age for partial resurfacing was 53years and 59 years for total joint replacements. Radiological analysis found that the antero-posterior standing views had only 66% sensitivity and 73% specificity for the presence of Grade 3 changes or worse in the medial compartment in comparison with Rosenberg views having a sensitivity of 73% and a specificity of 83%. Skyline views had a sensitivity of 56% and 100% specificity. This study indicates that a large proportion of cases may be suited to local and limited resurfacing. Cases suitable for Patello-femoral and Bi-compartmental replacements were very common, but with the patella-femoral joint's tendency to be more forgiving in terms of symptoms, meaning that indications for uni-compartmental replacement might well be much higher than the arthroscopic findings suggested. On the other hand, the presence of high levels of Grade 1 and 2 changes in other compartments seems to indicate a need for caution particularly in younger patients. This study also indicates a need for better methods of assessing local cartilage health such as enhanced MRI scanning or spectroscopy.
Operative approaches to the acetabulum are generally classified into anterior, posterior, extensile or combined approaches. The choice of approach depends upon the fracture pattern and the amount of relative displacement affecting the anterior and posterior bony structures. Occasionally, extensile or combined surgical approaches are indicated for the treatment of complex fracture patterns with extensive involvement of both the anterior and posterior acetabular anatomy. However, it is believed that these approaches may be associated with higher complication rates than more limited surgical approaches. The ilioinguinal approach described by Letournel is routinely employed in the treatment of anterior column, anterior wall, anterior column/posterior hemi- transverse and certain associated both-columns, transverse and T-type fractures. The utility of this approach is sometimes limited by difficulty in visualising, reducing and applying instrumentation to the quadrilateral plate and posterior column components of these fractures. A surgical approach described by Stoppa in 1989—and later extended to acetabular indications by Cole and Bolhofner—can be used, often in combination with the lateral window of the standard ilioinguinal approach, to effectively treat the same range of fractures as an ilioinguinal approach. Access to the quadrilateral plate and certain displaced posterior column fracture lines is enhanced by this approach—possibly eliminating the need for combined or extensile approaches in certain cases. A retrospective study undertaken at our institution demonstrated that anatomic articular reduction was achieved in 14 of 17 complex acetabular fractures treated via a Stoppa approach. All fractures in the study had at least 5 mm of posterior column displacement preoperatively.
Abstract. Extended Trochanteric Osteotomy (ETO) improves surgical exposure and aids femoral stem and bone cement removal in Revision Total Hip Replacement (RTHR) surgery. The aim of this study was to identify healing rates and complications of ETO in RTHR. Methods. From 2012 to 2019 we identified patients who underwent ETO for RTHR. Data collected demographics, BMI, diabetes, anticoagulants, indication for ETO, surgical approach, length of ETO and complications. Descriptive analysis of patient demographics, multiple linear regression analysis was performed to assess ETO complications. Results. There were 63 patients with an average age of 69 years.
Introduction. The management of fracture-related infection has undergone radical progress following the development of international guidelines. However, there is limited consideration to the realities of healthcare in low-resource environments due to a lack of available evidence in the literature from these settings. Initial antimicrobial suppression to support fracture union is frequently used in low- and middle-income countries despite the lack of published clinical evidence to support its practice. This study aimed to evaluate the outcomes following initial antimicrobial suppression to support fracture union in the management of fracture-related infection. Materials & Methods. A retrospective review of consecutive patients treated with initial antimicrobial suppression to support fracture healing followed by definitive eradication surgery to manage fracture-related infections following intramedullary fixation was performed.
Introduction. The purpose of this study was to analyse the efficacy and complications associated with the use of Calcium Sulphate synthetic bone graft in a paediatric population. There are no published articles on the use in children. Materials & Methods. A retrospective review was undertaken of the notes, microbiology, and X-Rays of 17 cases (in 15 patients) of calcium sulphate use in paediatric patients. As well as patient demographic data, data collected included indication, use of additional agents (antibiotics), return to theatre, and wound complications. Major complications were also assessed for. Results. There were 17 cases, in 15 patients, in our case series where calcium sulphate synthetic bone graft was used. The average patient age was 12.0 years (range 5 years – 17 years).
Tibiotalocalcaneal arthrodesis (TTCA) is a salvage procedure to functionally block the ankle and subtalar joints to treat severe ankle and hindfoot disease. Complication rates range between 20-40%, with below-knee amputation rate 1.5-19%. The heterogeneous patient cohort and indication(s) for surgery make post-operative outcomes difficult to predict. Consulting patients about potential pain and functional outcomes is difficult. When researching this area commonly, PROM data is used, less frequently functional objective measures like the 2-minute-walk-test (2MWT) are used. The aim was to compare the PROMs and objective outcomes between patients who underwent TTCA and BKA. Fifty-two patients underwent a TTCA using an intramedullary hindfoot nail.
