Reports of improved functional outcome of Metal on Metal Hip Resurfacing Arthroplasty (mHRA) to Total Hip Replacement needs to be balanced with concerns of metal ion release. By removing cobalt-chrome, cHRA reduces these risks. To the author's knowledge, there is no data available on functional outcomes of cHRA, therefore the aim of the study was to compare the function between cHRA patients and mHRA patients. 24 patients received a unilateral cHRA (H1, Embody) and was compared to 24 age and gender matched patients with a unilateral mHRA (BHR, Smith and Nephew). All patients completed the Oxford Hip Score (OHS)[T2] and underwent gait analysis on an instrumented treadmill before and at a mean of 74wks (+/− 10) for mHRA and 53wks (+/− 2) for cHRA post op. Walking trials started at 4km/h and increased in 0.5km/h increments until a top walking speed (TWS) was achieved. Vertical ground reaction forces (GRF) were recorded along with the symmetry index (SI). Spatiotemporal measures of gait were also recorded. Vertical GRF were captured for the entire normalised stance phase using statistical parametric mapping (SPM; CI = 95%). The gain in OHS was similar: H1 (25-46), BHR(27-47). TWS increased by 19% with H1 (6.02 – 8.0km/hr), and 20% with BHR (6.02 – 7.37km/hr). SPM of the entire gait cycle illustrated the restoration of symmetry in both groups with no difference in GRF across the stance phase between groups at 5km/hr pre-op and post-op. At faster speeds (6.5km/hr), H1 patients had a mid-support GRF slightly closer to normal compared to BHR. Both groups increased step length similar from pre to post op (H1:0.76 – 0.85cm, BHR:0.77-0.86cm). In this study, subjective and objective functional outcome measures suggest that short term functional outcomes of ceramic resurfacing is not inferior to metal resurfacing.

The treatment of osteonecrosis of the femoral head (ONFH) in young and active patients remains a challenge. The purpose of this study was to determine and compare the clinical and radiographic results of the two different hip resurfacing systems; hemi-resurfacing and metal-on-metal total hip resurfacing in patients with ONFH. This study was a retrospective review of 20 patients with 30 hips who had ONFH and underwent hemi-resurfacing or total hip resurfacing between November 2002 and February 2006. We mainly performed hemi-resurfacing for early stage ONFH, and total hip resurfacing for advanced stages. Fifteen hips in 11 patients had a hemi-resurfacing component (Conserve, Wright Medical Co) with a mean age at operation of 50 years and an average follow-up of 5.5 years. Fifteen hips in 10 patients had a metal-on-metal total hip resurfacing component (Birmingham hip resurfacing, Smith & Nephew Co.) with a mean age at operation of 40 years and an average follow-up years.Introduction

The renewed interest in the clinically proven low wear of the metal-on-metal bearing combined with the capacity of inserting a thin walled cementless acetabular component has fostered the reintroduction of hip resurfacing. Although hip replacement and resurfacing procedures both aim to restore mobility, improve joint function, and relieve pain, it is unclear how each differ in terms of gait mechanics and if they are affected by varying walking speeds. We compared limb symmetry and ground reaction force (GRF) profiles between bilateral total hip arthroplasty patients (THA), bilateral hip resurfacing arthroplasty patients (HRA), and healthy control participants (CON) during level-treadmill walking at different speeds. Bilateral THA and bilateral HRA patients (nTHA = 15; nHRA = 15; postoperative 12–18 months), and age-, mass-, and height-matched CON participants (nCON = 20) underwent gait analysis on an instrumented treadmill. Walking trials started at 4 km/h and increased in 0.5 km/h increments until top walking speed (TWS) was achieved. Patients undergoing hip resurfacing arthroplasty (HRA) is typically reserved for highly active patients. Patient Reported Outcome Measures (PROMs) such as the Oxford Hip Score (OHS) are reported to have ceiling effects, which may limit physicians' ability to measure health gain in these patients. The Metabolic Equivalent of Task (MET) index is a validated compendium assigning energy expenditure to a wide range of activities; for example, a slow walk expends 2.9 kcal/kg/hour, golf expends 4.0 kcal/kg/hour, while moderate lacrosse typically expends 8.1 kcal/kg/hour. We hypothesized that for patients with high OHS (47–48) after HRA, the MET index could better discriminate between high-performing individuals. We evaluated 97 consecutive HRA patients performed by a single surgeon. They prospectively completed an online Oxford Hip Score. They also listed three activities which they had performed independently in the preceding 2 weeks with a Likert-scale slider denoting intensity of effort. Matched data-sets were obtained from 51 patients, from which 23 had OHS of 47–48 at 6-months. Their activity with the highest MET index was selected for analysis. The 23 patients' OHS improved from 29.3 ± 7.0 preoperatively to 47.6 ± 0.5 after 6-months, while their MET indices improved from 8.5 ± 3.7 to 12.9 ± 3.5 kcal/kg/hr. The activities performed by these high-performance individuals ranged from the lowest, pilates (8.05 kcal/kg/hour), to highest, running at 22km/hr (23 kcal/kg/hour). 45% of patients undergoing HRA in this cohort had OHS of 47 and 48 at 6-months after surgery. Unlike the OHS, the MET index described variation in physical activity in these high-performance individuals, and did so on an objective measurable scale.
Hip resurfacing arthroplasty (HRA) became a popular procedure in the early 90s because of the improved wear characteristic, preserving nature of the procedure and the optimal stability and range of motion. Concerns raised since 2004 when metal ions were seen in blood and urine of patients with a MoM implant. Design of the prosthesis, acetabular component malpositioning, contact-patch-to-rim distance (CPR) and a reduced joint size all seem to play a role in elevated metal ion concentrations. Little is known about the influence of physical activity (PA) on metal ion concentrations. Implant wear is thought to be a function of use and thus of patient activity levels. Wear of polyethylene acetabular bearings was positively correlated with patient's activity in previous studies. It is hypothesized that daily habitual physical activity of patients with a unilateral resurfacing prosthesis, measured by an activity monitor, is associated with habitual physical activity. A prospective, explorative study was conducted. Only patients with a unilateral hip resurfacing prosthesis and a follow-up of 10 ± 1 years were included. Metal ion concentrations were determined using ICP-MS. Habitual physical activity of subjects was measured in daily living using an acceleration-based activity monitor. Outcome consisted of quantitative and qualitative activity parameters. In total, 16 patients were included. 12 males (75%) and 4 females (25%) with a median age at surgery of 55.5 ± 9.7 years [43.0 – 67.9] and median follow-up of 9.9 ± 1.0 years [9.1 – 10.9]. The median cobalt and chromium ion concentrations were 25 ± 13 and 38 ± 28 nmol/L. A significant relationship, when adjusting for age at surgery, BMI, cup size and cup inclination, between sit-stand transfers (p = .034) and high intensity peaks (p = .001) with cobalt ion concentrations were found (linear regression analysis). This study showed that a high number of sit-stand transfers and a high number of high intensity peaks is significantly correlated with high metal ion concentrations, but results should be interpreted with care. For patients it seems save to engage in activities with low intensity peaks like walking or cycling without triggering critical wear or metal ions being able to achieve important general health benefits and quality of life, although the quality (high intensity peaks) of physical activity and behaviour of patients (sit-stand-transfers) seem to influence metal ion concentrations.
Metal-on-metal hip implants can produce adverse tissue reactions to wear debris. Increased metal ion concentrations in the blood are measured as a proxy to wear and the complications it can trigger. Many studies have examined various factors influencing the metal ion concentrations. This is the first study to investigate the effect of physical activity level, as objectively measured in daily life, on blood ion levels, expecting higher concentrations for higher patient activity. Thirty-three patients (13F/20M, 55.8 ± 6.2 years at surgery) with a unilateral resurfacing hip prosthesis were included. At last follow-up (6.8 ± 1.5 years) cobalt and chromium concentrations in the blood were determined by inductively coupled plasma mass spectrometry. Physical activity was measured during 4 successive days using a 3D-acceleration-based activity monitor. Data was analysed using validated algorithms, producing quantitative and qualitative parameters. Acetabular cup position was measured radiographically. Hip resurfacing has resurged in the last decade due to a renewed interest in metal on metal bearing. One of the proposed advantages is ease of revision of the femoral component. Short term functional results after femoral revision are similar to those after conventional total hip replacement. Survival and function after revision of the acetabular component only or of both components have not been reported. We aimed to assess hip function and implant survival after revision of the acetabular component for failed Birmingham hip resurfacing (BHR). The Oswestry Outcome Centre collected data prospectively on 5000 patients who underwent hip resurfacing between 1997 and 2002. Of these, 182 hips were revised: 42% had revision of the femoral component only, 8% revision of the acetabular component only, and 50% revision of both components. This study analyzed patients who had revision of the acetabular component, either in isolation or in combination with the femoral component.Background

The use of hip resurfacing arthroplasty (HRA) has largely regressed due to the fear of metal-on-metal bearings. However committed HRA users continue to assert the functional advantages that a geometry retaining implant would have on a patient”s hip. Currently worldwide, HRA is only recommended to men who demand an active lifestyle. Despite this precarious indication, it is not clear to what extent HRA has on higher activity function. The aim of this study was to determine the functional extent to which could be achieved with HRA. The primary objective is to assess the loading pattern change for patients implanted with HRA at high walking speeds and inclinations. The second objective is to compare their loading features to a healthy group to determine if a normal gait pattern could be achieved. Between 2012 and 2016, a total of 28 prospective unilateral HRA patients were analysed on an instrumented treadmill from a single centre. All 28 patient patients had a uniform implant type and had no other lower limb operations or disease. Perioperative plain orthogonal radiographs were used to measure hip length and global hip offset change. A healthy control group (n=35) were analysed to compare. All HRA patients gait characteristics were assessed at incrementally higher speeds and inclinations to determine the extent of improvement HRA has on a challenging activity. A Student t-test along with a multivariate analysis was done with significance set at α=0.05. Weight and height variance was accounted with Hof normalisation. The HRA and control group were reasonably matched for age (57 vs 55yrs), BMI (27 vs 25) and height (175 vs 170cm) respectively. Hip measurements revealed less than 5mm change for all cases. The mean time from initial preoperative gait assessment to postoperative assessment was 30 months (24–48months). The mean top walking speed for controls was 1.97m/s and postoperatively 2.1 m/sec for the HRA group. The significant (p<0.001) loading change during flat walking can be seen with restoration of symmetry. Walking at an inclination demonstrated a marked change during weight acceptance (p<0.001) and a loading pattern returning to near normal. This prospective study found HRA patients walking faster than age matched controls. They demonstrated a significant change in their loading pattern, by significantly shifting load from the unaffected side to the implanted side. Uphill walking, an activity which requires more hip flexion, demonstrated a change in stance phase which was near normal. This small comparative study confirms near physiological function can be achieved with HRA at higher activity levels.
Femoral neck fractures remain the leading cause of early failure after metal-on-metal hip resurfacing. Although its' exact pathomechanism has yet to be fully elucidated, current retrieval analysis has shown that either an osteonecrotic event and/or significant surgical trauma to the femoral head neck junction are the leading causes. It is most likely that no single factor like patient selection and/or femoral component orientation can fully avoid their occurrence. As in osteonecrosis of the native hip joint, a certain cell injury threshold may have to be reached in order for femoral neck fracture to occur. These insults are not limited to the surgical approach, but also include femoral head preparation, neck notching, and cement penetration. Although some have argued that the posterior approach does not represent an increased risk fracture for ON after hip resurfacing because of the so-called intraosseous blood supply to the femoral head, to date, the current body of literature on femoral head blood flow in the presence of arthritis has confirmed the critical role of the extraosseous blood supply from the ascending branch of the medial circumflex, as well as the lack of any substantial intraosseous blood supply. Conversely, anterior hip dislocation of both the native hip joint as well as the arthritic hip preserves femoral head vascularity. The blood supply can be compromised by either sacrificing the main branch of the ascending medial femoral circumflex artery or damaging the retinacular vessels at the femoral head-neck junction. Thus an approach which preserves head vascularity, while minimizing soft tissue disruption would certainly be favorable for hip resurfacing. This presentation will review the current state of knowledge on vascularity of the femoral head as well as surgical techniques enhancing its preservation.
Wear debris induced osteolysis is a recognized complication in conventional metal-on-polyethylene hip arthroplasty. One method of achieving wear reduction is through the use of metal-on-metal articulations. One of the latest manifestations of this biomaterial combination is in designs of hip resurfacing which are aimed at younger, more active patients. But, do these metal-on-metal hip resurfacings show low wear when implanted into patients? Using a Mitutoyo Legex 322 co-ordinate measuring machine (scanning accuracy less than 1 micron) and a bespoke computer program, volumetric wear measurements for retrieved Articular Surface Replacements (ASR, DePuy) metal-on-metal hip resurfacings were undertaken. Measurements were validated against gravimetric calculations for volumetric wear using a sample femoral head that was artificially worn in vitro. At 5mm3, 10mm3, and 15mm3 of material removal, the method was shown to be accurate to within 0.5mm3. Thirty-two femoral heads and twenty-two acetabular cups were measured. Acetabular cups exhibited mean volumetric wear of 29.00mm3 (range 1.35 - 109.72mm3) and a wear rate of 11.02mm3/year (range 0.30 - 63.59mm3/year). Femoral heads exhibited mean wear of 22.41mm3 (range 0.72 - 134.22mm3) and a wear rate of 8.72mm3/year (range 0.21 - 31.91mm3/year). In the 22 cases where both head and cup from the same prosthesis were available, mean total wear rates of 21.66mm3/year (range 0.51 - 95.50mm3/year) were observed. Revision was necessitated by one of five effects; early femoral neck fracture (4 heads), avascular necrosis (AVN) (2 heads, 1 cup), infection (1 head, 1 cup), adverse reaction to metal debris (ARMD) (19 heads, 18 cups) or ARMD fracture (6 heads, 2 cups). Mean paired wear rates for the AVN and infection retrievals were 0.51mm3/year and 3.98mm3/year respectively. In vitro tests typically offer wear rates for metal-on-metal devices in the region of 2-4mm3. Mean paired wear rates for ARMD and ARMD fracture were 17.64mm3/year and 68.5mm3/year respectively, significantly greater than those expected from in vitro tests. In the 4 cases of early fracture, only the heads were revised so a combined wear rate calculation was not possible. The heads exhibited mean wear rate of 8.26mm3/year. These high wear rates are of concern.
Hip resurfacing has generally been used in younger patients with early osteoarthritis of the hip. There has been considerable recent interest in this over the past few years. We conducted a prospective randomised trial comparing 2 hip resurfacing implants, Durom and ASR looking at radiological and clinical outcomes. Forty-nine patients (78% male) with hip osteoarthritis which met the criteria for hip resurfacing were randomised to receive either a Durom or ASR resurfacing implant. These patients have so far been followed up for a minimum of one year. The groups were comparable in age (p=0.124) and gender (p=0.675). The average age in the ASR group was 54.04 years and in the Durom group it was 51.25. Radiological views were scrutinised immediately post op and at final follow up so far to look at cup inclination, stem-shaft angle, and acetabular osseointegration. Clinical outcomes were compared using the Oxford hip scores, WOMAC scores and SF12 scores. At minimum follow up of 1 year the mean post operative Oxford hip score was not significantly different between the Durom (45.32, SD 3.93) and ASR (43.44, SD 8.44). The mean post operative WOMAC score was also not significantly different between the Durom (52.56, SD 6.06) and ASR (49.63, SD 2.23). There was no difference between the groups with regards to signs of osseointegration from radiological assessment (p=0.368). There were 3 periprosthetic femoral neck fractures (5.7%) and one revision for pain. We conclude from this trial that there is no difference in the clinical or radiological findings between the Durom and ASR implants.
The reintroduction of hip resurfacing has raised the possibility of whether it could offer a better outcome in sporting and work activity after surgery compared to total hip replacement. Questionnaires were analysed from 125 total hip replacement (THR) and 108 hip resurfacing (HR) patients regarding participation in sport in the year after their surgery and returning to work they were employed at prior to their surgery. The two groups had similar average age (61 vs 60) and pre-operative Oxford hip scores (41 vs 38). Seventy-one of one-hundred and twenty-five THR patients compared to 76/108 HR patients participated in sporting activity in the year after their surgery. When including only patients that played sport before their surgery 54/75 THR and 66/83 HR patients returned to same sporting activity level after their surgery. Of the patients that worked before surgery 35/44 THR patients compared to 70/74 HR patients returned work to after their surgery. There is significant difference quantity of patients participating in sporting activity after HR and THR (fishers exact test, p value=0.04). However there is no significant difference in quantity of patients returning to sporting activity after HR or THR including only patients that played sport before their surgery (fishers exact test, p value=0.35). There is a significant difference in the quantity of patients returning to work activity after HR and THR (fishers exact test, p value= 0.02). HR has a better outcome in patients participating in sport after surgery compared to THR however there is no benefit for patients that are active pre-operatively. More patients return to work after HR than THR.
Introduction of new implants has been widely debated lately, mainly in response to the problems surrounding hip resurfacing and one company recalling its product. Medicines and Healthcare products Regulatory Agency (MHRA) also issued advice about the management and monitoring of patients implanted with metal-on-metal articulations. In response to MHRA advice all the patients who underwent hip resurfacing in our hospital were assessed by two consultants according to MHRA guidelines. Here we present the findings from our District General Hospital. Between November 2006 and March 2009 we performed 42 hip resurfacings in 39 patients. Nearly all the procedures were performed by a single surgeon (39 out of 42). There were 27 males (28 hips) and 12 females (14 hips). Mean age of the patients at the time of surgery was 55.6 years (age range 40–67 years). Patients were followed up for a mean of 49 months (range 33–66 months). 27 patients (28 hips) were asymptomatic at the time of last follow up. Serum cobalt and chromium ion levels were measured in 21 patients (24 hips). Both the ion levels were within the recommended levels in 20 patients and over the recommended limit in 2 patients (one with bilateral hip resurfacing). MRI was diagnostic in 2 out of 5 patients. 5 patients (7 hips) were revised (one hip for neck resorption, 4 hips in 3 patients with lesions on MRI, 1 patient with bilateral hips for elevated serum ion levels) Considering the revision rate (7 hips out of 42 hips, 16%), we do believe that the review of the hip resurfacings in spite of the controversies surrounding the diagnostic criteria is necessary. This group of patients need to have continued surveillance, preferably by a select group of surgeons.
We have previously observed an increase in total bone mineral density and reduced bone turnover (TRAP5b and osteocalcin) in patients with well-functioning metal-on-metal hip resurfacing (MOMHR). Here, we provide data to support the hypothesis that osteoclast differentiation and function is altered in this patient population, and that this effect is transferrable through their serum. Patients with well-functioning MOMHR (cases, n=18) at a median follow-up of 8 years were individually matched for gender, age and time-since-surgery to a low-exposure group consisting of patients with THA (controls, n=18). The monocyte fraction of patient peripheral blood was isolated and differentiated into osteoclasts on dentine wafers using RANKL and M-CSF supplemented media (osteoclastogenic media, OM). Cultures were monitored for the onset of resorption, at which point the cells were treated with OM, autologous serum or serum from matched MOMHR/THA donors, all supplemented with RANKL and M-CSF. At the end of the culture, cells were TRAP-stained and quantified using CellD Software Package, Olympus. When cells were differentiated in standard osteoclastogenic media, the resorbing ability of osteoclasts derived from MOMHR patients was reduced 22%(p<0.0079) compared to THA. The resorbing ability of osteoclasts generated from MOMHR patients and differentiated in autologous serum was reduced 33%(p<0.0001), whilst matched THA serum caused a smaller reduction of 14%(p<0.01). When cells derived from THA patients were differentiated in autologous serum, the resorbing ability of osteoclasts was similarly reduced by 35%(p<0.0001), whilst the matched MOMHR serum also caused a reduction of 21%(p<0.0001). This data suggests that prior exposure to higher circulating Co and Cr in patients with MOMHR reduces osteoclastogenesis, and that the detrimental effect on the functionality of mature osteoclasts is transferable through the serum. This has implications for systemic bone health of patients with MOMHR or modular taper junctions.
Metal-on-metal (MOM) hip arthroplasty, including resurfacing, has become the subject of recent research and debate. There is the perceived benefit of improved wear rates of bearing surfaces leading to superior durability and performance of these types of implant. An associated feature of MOM bearing surfaces is the generation of metal ions. These can have local and systemic cytotoxic effects. An immunoloigical response has been suggested, however, metal wear debris may cause direct damage to cellular DNA. Studies have shown that release of these ions is related to bearing diameter and component alignment. However, little is known about the relationship between metal ion levels and implant survivorship. The MHRA has published guidelines on the follow-up of patients with MOM implants including measurement of serum ion levels and cross sectional imaging. Between February 2001 and November 2009, 135 patients (164 hips) had MOM resurfacing arthroplasty at our institution. We report a retrospective analysis of the data generated by review of these patients. Of the 135 patients, 91 were identified for clinical review. Each patient had serum metal ion levels measured, plain AP radiographs of the pelvis examined and, in the presence of raised metal ions, a Metal Artefact Reduction Sequence (MARS) MRI performed. 27 patients (35 hips) had raised metal ion levels (Cobalt and Chromium). Patients with raised metal ion levels had a mean acetabular cup inclination of 52.7 degrees compared with a mean inclination of 48.6 degrees in patients with normal ion levels (p<0.05). MARS MRI in the raised ion group revealed 9 patients with appearances suggestive of ALVAL. A number of these patients had hip revision surgery with the remainder awaiting potential revision. These findings reflect current evidence suggesting a relationship between sub-optimal component position and raised metal ion levels and an increased rate of ALVAL.
The friction and lubrication behaviour of four Biomet ReCap components with a nominal diameter of 52 mm and diametral clearance ranging from 167-178 μm were investigated using a friction hip simulator. Friction testing was carried out using pure bovine serum and aqueous solutions of bovine serum (BS), with and without carboxymethyl cellulose (CMC), adjusted to a range of viscosities (0.001-0.236 Pas). The Stribeck analyses suggested mixed lubrication as the dominant mode with the lowest friction factor of 0.07 at a viscosity of 0.04 Pas. The femoral resurfacing systems provide an alternative to hemi and total hip arthroplasty and offer several unique advantages including large resurfacing heads (>35–60 mm diameter) allowing increased range of motion (and stability) over the traditional 28 mm artificial hip joints, with excellent tolerances and surface finish leading to a reduction in wear, as well as preserving primary bone with the femoral canal remaining untouched. This work has investigated the friction and lubrication behaviour of four 52 mm metal-on-metal Biomet ReCap components with a clearance of 167-178 μm using serum-based lubricants.ABSTRACT
INTRODUCTION
Hip resurfacing procedures have gained increasing popularity for younger, higher demand patients with degenerative hip pathologies. However, with concerns regarding revision rates and possible adverse metal hypersensitivity reactions with metal-on-metal articulations, some authors have questioned the hypothesised superiority of hip resurfacing over total hip arthroplasty. The purpose of this meta-analysis was to compare the clinical and radiological outcomes and complication rates of these two procedures. A systematic review was undertaken of all published and unpublished research up to January 2010. The primary search was of the databases Medline, CINAHL, AMED and EMBASE, searched via Ovid using MeSH terms and Boolian operators ‘hip’ AND ‘replacement’ OR ‘arthroplasty’ AND ‘resurfacing’. A secondary search of unpublished literature was conducted using the databases SIGLE, the National Technical Information Service, the National Research Register (UK), the British Library's Integrated Catalogue and Current Controlled Trials databases using the same search terms as the primary search. All included studies were critically appraised with the CASP appraisal tool. In total, 46 studies were identified from 1124 citations. These included 3799 hip resurfacings and 3282 total hip arthroplasties. On meta-analysis, functional outcomes for subjects following hip resurfacing were better than or the same as subjects with a total hip arthroplasty, with significantly higher WOMAC score (Mean Difference (MD)=−2.41; 95% Confidence Interval (CI): −3.88, −0.94; p=0.001), and significantly better Harris Hip Score (range of motion component) (MD=−0.05; 95% CI: (−0.07, −0.03; p<0.0001) and overall Harris Hip Score (MD=2.51; 95% CI: 1.24, 3.77; p=0.0001) in the hip resurfacing compared to total hip arthroplasty cohorts. In respect to these factors, the current evidence-base, whilst substantial in its size, may be questioned in respect to its quality in determining superiority of hip resurfacing over total hip arthroplasty.
Systemic concentrations of metal ions (cobalt and chromium) are persistently elevated in patients with metal-on-metal hip resurfacing (MOMHR) compared to conventional total hip arthroplasty (THA). Several studies by us and others have described the detrimental effects of metal exposure on survival and function of various cell types The cohort consisted of 34 patients with a well-functioning MOMHR at a median follow-up of 9.75 years. These were individually matched for gender, age and time-since-surgery to a non-exposure group consisting of patients with THA. Genomic DNA was isolated from blood samples and cell composition estimated using the ‘estimateCellCounts’ function in ‘minfi R-package’. Methylation was assessed using the Illumina 450k BeadChip array analysing 426,225 probes.
