Background. Cup inclination is a major factor in the success of a total hip replacement. An open cup position can lead to dislocation or increased wear from rim loading and a closed cup position lead to impingement against the femoral neck or psoas. Although the ideal inclination for cup position is recommended as between 40 and 45 degrees, accurate positioning of the implant might be influenced by pelvic flexion and movement of the patient's pelvis during the procedure. We wanted to examine if the transvers acetabular ligament (TAL) could be used to determine cup inclination intra-operatively. Methods. 16 hips from 9 cadaveric specimens were used for the study. A computer navigation system (Brain lab) was used to measure and document the exact inclination and version of the acetabular trial component in three positions: flush with the transvers acetabular ligament (TAL), with the rim of the cup 5 mm from the TAL in a cranial direction and with the rim of the cup 5 mm caudally displaced. Statistical analysis of the results was performed by the Department of Biostatistics. Findings. With the cup positioned flush with the TAL, the average version was 43 degrees (range 37 to 47 degrees.) When there was a 5 mm gap between the TAL and the cup the average inclination was 28 degrees (21 to 35 degrees.) When the cup was opened so it covered the TAL by 5 mm the average inclination increased to 64 degrees (55 to 75 degrees.) The average anteversion angle was 18 degrees (range 15 to 25 degrees.). Conclusion. We found the transverse acetabular ligament to be an accurate landmark for positioning of the femoral implant as far as version and inclination was concerned. We recommend positioning the acetabular component flush with the TAL as cup inclination was shown to be ideal in all cases when we adhered to that principle. NO DISCLOSURES

INTRODUCTION. Retrieval and clinical studies of metal-on-metal (MoM) bearings have associated increased wear. 1. and elevated patient ion levels. 2. with steep cup inclination angles and edge loading conditions. The University of Leeds have previously developed a hip simulator method that has been validated against retrievals and shown to replicate clinically relevant wear rates and wear mechanisms. 3,4. This method involves introducing lateral microseparation to represent adverse joint laxity and offset deficiency. This study aimed to investigate the effect of microseparation representing translational malpostion, and increased cup inclination angle, representing rotational malposition, in isolation and combined on the wear of different sizes (28 and 36mm) MoM bearing in total hip replacement (THRs). MATERIALS AND METHODS. The wear of size 28mm and 36mm MoM THRs bearings was determined under different in vitro conditions using the Leeds II hip simulator. For each size bearing, two clinical cup inclination angles were considered, 45° (n=3) and 65° (n=3). The first three million cycles were run under standard gait conditions and subsequently three million cycles were run under microseparation conditions. Standard gait cycles included a twin peak load (300N–3000N), extension/flexion (−15°/+30°) and internal/external rotation (±10°). Microseparation. 4. was achieved by applying a 0.4–0.5mm medial displacement to the cup relative to the head during the swing phase of the standard gait cycle resulting in edge loading at heel strike. The lubricant was 25% (v/v) new-born calf serum. The wear volume was determined through gravimetric analysis every million cycles. One way ANOVA was performed (significance: p<0.05), and 95% confidence limits were calculated. RESULTS. Under standard gait conditions, the 28mm MoM bearing showed head-rim contact and increased wear rate with increased cup inclination angle but the 36mm bearing did not show any increase in wear. Microseparation and edge loading increased the wear rate of MoM bearings for all cup inclination angle conditions and bearing sizes (Figure 1). DISCUSSION. With the larger size bearings, head-rim contact occurred at a steeper cup inclination angle (>65°) providing an advantage over smaller bearings. Under standard gait conditions, where head-rim contact did not occur, wear was low, due to mixed lubrication and wear reduction through a protein boundary film. However, edge loading of the cup, with elevated stress, caused excess damage and wear. This effect was more dominant with microseparation conditions to that of head-rim contact due to increased cup inclination angle alone. Under microseparation conditions, there were no significant differences in the wear rates of the 28mm and the 36mm size bearings. However, the wear rates obtained in this study for 28mm and 36mm bearings were significantly lower than those obtained for size 39mm surface replacement MoM bearings (8.99 mm. 3. /million cycles) tested under the same adverse conditions. 5. . CONCLUSION. This study shows the importance of acetabular cup design and correct surgical positioning of the femoral head and acetabular cup and restoration of offset and cup centre. ACKNOWLEDGEMENT. The purpose of this study was to examine the utility of the acetabular component introducer as a tool to intra-operatively predict implant inclination in total hip arthroplasty. This study investigated (1) the correlation between intra-operative photographic assessment of cup inclination using the acetabular introducer and that measured on post-operative radiograph; and (2) the accuracy of intra-operative prediction of abduction angle. For this study, we prospectively recruited 56 patients scheduled to receive primary hip arthroplasty from one of two senior surgeons. During the procedure, the lead surgeon provided a prediction of the abduction angle based on the alignment of the impactor attached to the cup in its final seated position. A standardized anteroposterior (AP) photograph was then taken of the acetabular impactor in situ. Abduction angles were measured by two observers on the photographs and post-operative AP pelvis radiographs. Linear regression was used to determine the correlation between the angle of the guide measured on the photographs and the actual position of the implant measured on the radiograph. Descriptive statistics were further used to analyze the accuracy of the intra-operative prediction as compared with the abduction angle measured on the photographs. Measurements of cup position made from post-operative radiographs were significantly correlated with the measurements as assessed by intra-operative photographs (r = 0.34, p = 0.00). Our findings demonstrate that radiological abduction angles tend to be greater than that assessed by intra-operative photographs by a mean of 5.6 degrees (SD = 6.6 degrees; 95% CI = 7.3 to 3.9 degrees). Conversely, surgeon prediction of cup inclination based on the acetabular introducer differed from the radiographic measurements by a mean of 6.8 degrees (SD = 8.7 degrees). There was good agreement between the two observers in both photographic and radiographic measurement (k = 0.95, k = 0.96, respectively). In conclusion, we found that the intra-operative photographic assessment of acetabular cup inclination by acetabular impactor alignment tends to underestimate the abduction angle by a mean of approximately 5 degrees. In addition, intra-operative surgeon estimation of acetabular inclination did not appear accurate in this study demonstrating that cup position should rely on additional visual cues beyond that captured in the anteroposterior view of the cup introducer

Introduction. Wear plays a key role in the clinical outcome of total hip replacements (THR). In addition, increased frictional moment can stress the implant interfaces which may lead to high torsional loadings in the intermodular taper junction (fretting) and cup loosening and to the development of noise (squeaking). Against the background of larger head diameters (increased range of motion and decreased risk of dislocation), the friction induced by the joint articulation is of particular interest. As of now, the investigation of friction with the use of relevant joint kinematics and loadings are limited to numerical studies. Experimental approaches use simplified models which do not take into consideration complex activities. Thus, with the aim of this study is the identification of articular frictional moments that consider critical in vivo loading conditions and kinematics as well as the clinical cup inclination, head size and clearance of ceramic-on-ceramic hip bearings. Materials and Methods. A standard hip simulator (Minibionix 852 with 4 DOF Hip setup, MTS, Eden Prairie, USA) was modified in order to allow for high-precision friction measurements during head-insert articulation in all 6 DOF (MC2.5D-500, AMTI, Boston, USA). Disturbing systemic effects have been minimized by using quasi frictionless aerostatic lateral force compensation (Eitzenberger, Wessobrunn, Germany) and cross talk compensation. Beside the standard protocoll for in vitro wear assessment (ISO 14242-1), more complex profiles from in vivo patient data (Heidelberg Motion Lab and Orthoload database) have been used: normal walking with different walking speeds and patient's weights, stairs up/down and start-stop conditions. All-ceramic bearings (Biolox delta, Ceramtec, Plochingen, Germany) have been orientated in clinically relevant cup inclinations (30, 45, 60 and 75 deg). For each head diameter (28, 36 and 48 mm) n=8 specimens have been devided in two groups: small and large clearance according to the manufacturer's specification. All tests were run at 37°C in diluted bovine serum (20 g/l protein content). Results and Discussion. For all continuously running activities (normal walking, stairs up/down and ISO standard), increased resulting frictional moments have been measured with larger head diameters and smaller clearances in a range of less than 6 Nm. This data corresponds well to the results of a well-lubricated ceramic-on-ceramic bearing from numerical studies. In addition, the initial breakaway torque after a short resting period (start-stop initiation) was increased, where the highest maximal moments have been measured with increasing resting durations and larger head diameters (large clearance: up to 11 Nm, small clearance: up to 20 Nm). Interestingly enough, not in all cases a negative effect on the resulting moment was seen with increasing cup inclination, even though no subluxation was induced. INTRODUCTION. Ceramic-on-ceramic hip replacements have generated great interest in recent years due to substantial improvements in manufacturing techniques and material properties. 1. Microseparation conditions that could occur due to several clinical factors such as head offset deficiency, medialised cup combined with laxity of soft tissue resulting in a translation malalignment, have been shown to cause edge loading, replicate clinically relevant wear mechanisms. 2,3. and increase the wear of ceramic-on-ceramic bearings. 3,4. The aim of this study was to investigate the influence of increasing the femoral head size on the wear of ceramic-on-ceramic bearings under several clinically relevant simulator conditions. MATERIALS AND METHODS. The wear of size 28mm and 36mm ceramic-on-ceramic bearings (BIOLOX® Delta, CeramTec, Germany) was determined under different in vitro conditions using the Leeds II hip simulator. For each size bearing, two clinical cup inclination angles were considered, 55° (n=3) and 65° (n=3) for the 28mm bearing and 45° (n=3) and 65° (n=3) for the 36mm bearing. The first two (28mm study) or three (36mm study) million cycles ran under standard gait conditions and a subsequent three million cycles ran under microseparation conditions. A standard gait cycle included a twin peak load (300N–3000N), extension/flexion (−15°/+30°) and internal/external rotation (±10°). Microseparation. 3. was achieved by applying a 0.4–0.5mm medial displacement to the cup relative to the head during the swing phase of the standard gait cycle resulting in edge loading at heel strike. The lubricant was 25% (v/v) new-born calf serum which was changed approximately every 333,000 cycles. The wear volume was ascertained through gravimetric analysis every million cycles. One way ANOVA was performed (significance: p<0.05), and 95% confidence limits were calculated. RESULTS AND DISCUSSION. The mean wear rate under standard gait conditions was 0.05mm. 3. / million cycles for the 28mm bearings and significantly lower (p=0.003) for the 36mm bearings (Figure 1) which could be due to improved lubrication regime. The wear of ceramic-on-ceramic bearings was not influenced by the increase in cup inclination angle for either bearing size (Figure 1). The introduction of microseparation into the gait cycle resulted in stripe wear on the femoral head with a corresponding wear area at the rim of the acetabular cup and significantly higher wear rates of the ceramic-on-ceramic bearings (Figure 2). The wear rate of BIOLOX® Delta bearings under microseparation conditions was still low (<0.25mm. 3. /million cycles) compared to the third generation alumina ceramic-on-ceramic bearings (1.84mm. 3. /million cycles). 4. under the same adverse conditions. Under microseparation conditions, the wear rate of size 36mm bearings was significantly higher (p=0.004) than that for size 28mm bearings. This was thought to be due to the larger contact area for the larger bearings and deprived lubrication under edge loading conditions. For both bearing sizes, the combination of both steep cup inclination angles and microseparation conditions did not increase the wear rates any further compared to microseparation conditions alone (Figure 3). This study shows the importance of surgical positioning of the femoral head and acetabular cup and the importance of testing new bearing materials and designs using these adverse simulator methods. Introduction. Variations in component position can lead to dynamic separation and edge loading conditions. In vitro methods have been developed to simulate edge loading conditions and replicate stripe wear, increased wear rate, and bimodal wear debris size distribution, as observed clinically [1, 2]. The aim of this study was to determine the effects of translational and rotational positioning on the occurrence of dynamic separation and severity of edge loading, and then investigate the wear rates under the most severe separation and edge loading conditions on an electromechanical hip joint simulator. Materials and Methods. A hip joint simulator (ProSim EM13, Simulation Solutions, UK) was set up with 36mm diameter ceramic-on-ceramic (BIOLOX®delta, PINNACLE®, DePuy Synthes, UK) hip replacements. Three axes of rotation conditions (ISO 14242-1 [3]) was applied to the femoral head. This study was in two parts. I) A biomechanical test was carried out at 45° (n=3) and 65° (n=3) cup inclination angles with 1, 2, 3 and 4 (mm) medial-lateral translational mismatch between the centres of the head and cup. The amount of dynamic separation displacement between the head and cup was measured using a position sensor. The severity of edge loading was determined from the area under the axial force and medial-lateral force outputs during the time of separation [4]. II) A wear test was carried out at 45° (n=6) and 65° (n=6) cup inclination angles for three million cycles with translational mismatch of 4mm between the head and cup. The lubricant used was diluted new-born calf serum (25% v/v). Volumetric wear measurements were undertaken at one million cycle intervals and mean wear rates were calculated with 95% confidence limits. Statistical analysis was carried out using ANOVA and a t-test with significance levels taken at p<0.05. Results. Dynamic separation increased significantly with 3mm (p<0.01) and 4mm (p<0.01) translational mismatch at a 45° cup inclination angle (Figure 1). At 65° the separation increased significantly as the translational mismatch increased from 1mm to 4mm (p<0.01). The most severe edge loading conditions occurred at a 65° cup inclination angle with 4mm of translational mismatch (p<0.01, Figure 2). Mean wear rates were greater at a 65° cup inclination angle compared with a 45° cup inclination angle (p<0.01, Figure 3). Conclusion. Different levels of rotational and translational mismatch affected the separation between the head and cup during gait. Higher levels of translational mismatch and a steeper cup inclination angle may lead to more severe edge loading conditions and increased wear of ceramic-on-ceramic bearings in vivo. A new preclinical testing approach was developed to study the effects of edge loading due to variations in rotational and translational surgical positioning under ISO loading and angular displacement conditions. The first stage comprised of biomechanical tests to determine the occurrence and severity of edge loading in a range of component positions. The second stage investigated the tribological performance of the bearing surface under the worst case edge loading conditions

Introduction. Increased wear rates [1, 2] and acetabular rim fracture [3] of hip replacement bearings reported clinically have been associated with edge loading, which could occur due to rotational and/or translational mal-positioning [4]. Surgical mal-positioning can lead to dynamic microseparation mechanisms resulting in edge loading conditions. In vitro microseparation conditions have replicated stripe wear and the bi-modal wear debris distribution observed clinically [5, 6]. The aim of this study was to investigate the effect of steep cup inclination, representing rotational mal-positioning, on the magnitude of dynamic microseparation, severity of edge loading, and the resulting wear rate of a ceramic-on-ceramic bearing, under surgical translational mal-positioning conditions. Materials and Methods. Ceramic-on-ceramic bearings where the ceramic liner was inserted into a titanium alloy cup (BIOLOX® delta and Pinnacle® respectively, DePuy Synthes, UK) were tested on the six-station Leeds II hip simulator. The first test was run with the cups inclined at an angle equivalent, clinically, to 45° (n=6) and the second test was run with the cups inclined at an angle equivalent, clinically, to 65° (n=6). A standard gait cycle was run. A fixed surgical translational mal-positioning of 4mm between the centres of rotations of the head and the cup in the medial/lateral axis was applied on all stations. Both tests ran for three million cycles each. The lubricant used was 25% new-born calf serum. Wear was assessed gravimetrically using a microbalance (XP205, Mettler Toledo, UK) and geometrically using a coordinate measuring machine (CMM, Legex 322, Mitutoyo, UK). Statistical analysis was done using one way ANOVA with significance taken at p<0.05. Results. The magnitude of dynamic microseparation was significantly (p<0.01) higher when the inclination angle of the cup was steeper (Figure 1) under the same level of translational mal-positioning of 4mm. This has resulted in significantly (p<0.01) higher wear rates of 1.01mm. 3. /million cycles for the steep cup inclination group of 65° compared to 0.32mm. 3. /million cycles for the 45° inclined cups group (Figure 2). Furthermore, the penetration on the femoral heads was significantly (p<0.01) higher for the steep cup inclination angle group with a mean (±95% confidence limit) penetration of 33±6µm under the 65° cup inclination angle condition and 15±3µm under the 45° cup inclination angle condition (Figure 3). Discussion and Conclusion. This study showed that cup inclination angle affects the magnitude of dynamic microseparation for a given surgical translational mal-position, thus leading to severe edge loading and increased wear rates with increased cup inclination angles. The occurrence and severity of the resulting edge loading causing increased wear in hip bearings will depend on the combinations of surgical variations, such as steep inclination angle, excessive version angle, medialised cups, head offset deficiencies, stem subsidence, and joint laxity. Future work will include studying the effect of these variables on the level of dynamic microseparation, severity of edge loading, the offset frictional torque and level of resulting wear

Introduction. Many authors have described component position and leg length discrepancy (LLD) after total hip arthroplasty (THA) as the most important factors for good postoperative outcomes. However, regarding the relationships between component position and different approaches for THA, the optimal approach for component position and LLD remains unknown. The aims of this study were to compare these factors among the direct anterior, posterolateral, and direct lateral approaches on postoperative radiographs retrospectively, and determine which approach leads to good orientation in THA. Methods. We retrospectively evaluated 150 patients who underwent unilateral primary THA in our department between January 2009 and December 2014, with the direct anterior, posterolateral, or direct lateral approach used in 50 patients each. Patients with significant hip dysplasia (Crowe 3 or 4), advanced erosive arthritis, prevented osteotomy of the contralateral hip, and body mass index (BMI) of more than 30 were excluded. The mean age, sex, and preoperative diagnosis of the affected hip were equally distributed in patients who underwent THA with the different approaches. The mean BMI did not differ significantly among the groups. The radiographic measurements included cup inclination angle, dispersion of cup inclination from 40°, and LLD on an anteroposterior pelvic radiograph, and cup anteversion angle and dispersion of cup anteversion from 20° on a cross-table lateral radiograph postoperatively. We also measured the ratios of patients with both cup inclination of 30–50° and cup anteversion of 10–30° (target zone in our department), femoral stem varus/valgus, and LLD of 10 mm or less. Statistical analyses used an unpaired t-test and Fisher's exact test, with significance set at p<0.05. Results. The mean cup inclination was 36.9±5.1° for direct anterior approach, 40.8±7.5° for posterolateral approach, and 38.5±7.5° for direct lateral approach. Dispersion of cup inclination from 40° was almost identical in the three groups, with no significant differences. The mean cup anteversion was 23.4±5.5° for direct anterior approach, 25.9±9.2° for posterolateral approach, and 24.3±8.6° for direct lateral approach. Dispersion of cup anteversion from 20° differed between direct anterior approach and posterolateral or direct lateral approach (P<0.05 for each). The mean LLD was 1.3±6.6mm for direct anterior approach, 3.0±8.6mm for posterolateral approach, and 2.6±7.4mm for direct lateral approach. The mean LLD did not differ significantly among the three groups. The ratio of patients with both cup inclination of 30–50° and cup anteversion of 10–30° was significantly better for direct anterior approach than for posterolateral or direct lateral approach (78% vs. 52% and 52%, respectively; p<0.05). The ratios of femoral stem varus/valgus and LLD of 10 mm or less did not differ among the groups. Conclusions. The direct anterior approach in THA appeared to have small dispersion of cup anteversion angle and high ratio of cup component position in our target zone compared with the posterolateral and direct lateral approaches. However, the LLD and femoral stem varus/valgus after THA did not differ significantly among the three approaches postoperatively

Introduction. Component positioning is of great importance in total hip arthroplasty (THA) and navigation systems can help guide surgeons in the optimal placement of the implants. We report on a newly developed navigation system which employs an inertial measurement unit (IMU) to measure acetabular cup inclination and anteversion. Aims. To assess the accuracy of the IMU when used for acetabular cup placement and compare this with an established optical navigation system (ONS). Methods. At the time of acetabular cup impaction, the IMU and ONS were separately mounted on the impactor handle. Cup inclination and anteversion as measured by each device were recorded. Post-operative CTs were acquired for all patients and used to determine the final cup position. Results. Data were recorded for a total of 100 patients undergoing THA; 51 had a direct anterior approach (DAA) and 49 had a posterior approach (PA). In the DAA group, the mean difference in IMU versus CT measured cup inclination was −0.7°(range −6 to 8º) compared with mean difference of ONS versus CT of −2° (range −8 to 5º). Mean difference in IMU versus CT measured anteversion was − 1.3° (range −10 to 10º) compared with a mean difference of −1.1° (range −23 to 20º) between ONS and CT. In the PA group, mean difference in IMU versus CT inclination was 1.3º (range −8 to 6º) compared with mean difference between ONS versus CT of 1.6° (range −5 to 7º). Mean difference in anteversion was 3.7° (range −7 to 16º) between IMU and CT and 7.3° (range −3 to 19º) between ONS and CT. Conclusion. The novel IMU can be used to accurately determine the position of the acetabular cup at the point of impaction, demonstrating comparable accuracy with an established navigation system in the direct anterior approach, and even greater accuracy in the posterior approach

Hip resurfacing arthroplasty (HRA) is a bone conserving alternative to total hip arthroplasty. We present the early 1 and 2-year clinical and radiographical follow-up of a novel ceramic-on-ceramic (CoC) HRA in a multi-centric Australian cohort. Patient undergoing HRA between September 2018 and April 2021 were prospectively included. Patient-reported outcome measures (PROMS) in the form of the Forgotten Joint Score (FJS), HOOS Jr, WOMAC, Oxford Hip Score (OHS) and UCLA Activity Score were collected preoperatively and at 1- and 2-years post-operation. Serial radiographs were assessed for migration, component alignment, evidence of osteolysis/loosening and heterotopic ossification formation. 209 patients were identified of which 106 reached 2-year follow-up. Of these, 187 completed PROMS at 1 year and 90 at 2 years. There was significant improvement in HOOS (p< 0.001) and OHS (p< 0.001) between the pre-operative, 1-year and 2-years outcomes. Patients also reported improved pain (p<0.001), function (p<0.001) and reduced stiffness (p<0.001) as measured by the WOMAC score. Patients had improved activity scores on the UCLA Active Score (p<0.001) with 53% reporting return to impact activity at 2 years. FJS at 1 and 2-years were not significantly different (p=0.38). There was no migration, osteolysis or loosening of any of the implants. The mean acetabular cup inclination angle was 41.3° and the femoral component shaft angle was 137°. No fractures were reported over the 2-year follow-up with only 1 patient reporting a sciatic nerve palsy. There was early return to impact activities in more than half our patients at 2 years with no early clinical or radiological complications related to the implant. Longer term follow-up with increased patient numbers are required to restore surgeon confidence in HRA and expand the use of this novel product. In conclusion, CoC resurfacing at 2-years post-operation demonstrate promising results with satisfactory outcomes in all recorded PROMS

Imageless computer navigation systems have the potential to improve acetabular cup position in total hip arthroplasty (THA), thereby reducing the risk of revision surgery. This study aimed to evaluate the accuracy of three alternate registration planes in the supine surgical position generated using imageless navigation for patients undergoing THA via the direct anterior approach (DAA). Fifty-one participants who underwent a primary THA for osteoarthritis were assessed in the supine position using both optical and inertial sensor imageless navigation systems. Three registration planes were recorded: the anterior pelvic plane (APP) method, the anterior superior iliac spines (ASIS) functional method, and the Table Tilt (TT) functional method. Post-operative acetabular cup position was assessed using CT scans and converted to radiographic inclination and anteversion. Two repeated measures analysis of variance (ANOVA) and Bland-Altman plots were used to assess errors and agreement of the final cup position. For inclination, the mean absolute error was lower using the TT functional method (2.4°±1.7°) than the ASIS functional method (2.8°±1.7°, ρ = .17), and the ASIS anatomic method (3.7°±2.1, ρ < .001). For anteversion, the mean absolute error was significantly lower for the TT functional method (2.4°±1.8°) than the ASIS functional method (3.9°±3.2°, ρ = .005), and the ASIS anatomic method (9.1°±6.2°, ρ < .001). All measurements were within ± 10° for the TT method, but not the ASIS functional or APP methods. A functional registration plane is preferable to an anatomic reference plane to measure intra-operative acetabular cup inclination and anteversion accurately. Accuracy may be further improved by registering patient location using their position on the operating table rather than anatomic landmarks, particularly if a tighter target window of ± 5° is desired

Introduction. Computer navigation is a highly sophisticated tool in orthopedic surgery for component placement in total hip arthroplasty (THA). In order to apply it adequately it is of upmost importance that the targets the surgeon is trying to hit are well-defined. This concept considers all four component orientations: cup inclination (cIncl) and anteversion (cAV), stem antetorsion and neck-to-shaft angle. The optimising goal in this concept is maximising the size of the cSafe-Zone. Methods. A computerised 3D- model of a total hip prosthesis was used to systematically analyse all combinations of component orientations in automatised batch runs. Component orientations were varied for cup inclination, cup anteversion, neck antetorsion and neck inclination. Results. The combined Safe-Zone outlines spaces in a 3D-diagram that show the relationship between cup inclination, cup anteversion and neck anteversion, while the neck inclination is used as a curves parameter. These spaces include all component orientation that allow the predefined iROM without prosthetic impingement. In order to compare these results to Lewinnek's recommendation cross-sections were taken at distinct neck antetorsions in 5° intervals. Conclusion. The new combined Safe-Zone (cSafe-Zone) includes all orientation parameters of both total hip components and such gives well-defined recommendations for combined positioning of both components. Ideally it can be introduced into a smart computer navigation system in order to compute in real-time the best combined orientation of both components

The purpose of this preliminary study was to evaluate the feasibility and accuracy of HipAlign (OrthAlign, Inc., USA) system for cup orientation in total hip arthroplasty (THA). The subjects of this study were 5 hips that underwent primary cementless THA via a posterior approach in the lateral decubitus position. Evaluation 1; after reaming acetabular bone, a trial cup was placed in the reamed acetabulum in an aimed alignment using HipAlign. Then, the trial cup alignment was measured using HipAlign and CT-based navigation system in the radiographic definition. Evaluation 2; a cementless cup was placed in the reamed acetabular in an aimed alignment using CT-based navigation and cup alignment was measured using both methods. After operation, we measured the cup alignment using postoperative CT in each patient. In the results, the average cup inclination measured with HipAlign was around 5 degrees of true cup inclination angles. The average cup anteversion with HipAlign tended to be larger than that with CT-based navigation or postoperative CT in both evaluations. That is because there is a difference in the pelvic sagittal tilt between the lateral position and supine position. In conclusion, this study suggests that guiding cup alignment with the use of HipAlign is feasible through a posterior approach and the mean cup inclination measured with HipAlign showed an acceptable level of accuracy, but the mean cup anteversion is not reliable. We need a further modification for pelvic registration to improve the accuracy of cup anteversion

Introduction. Edge loading of hip replacements may result in plastic deformation, creep and wear at the rim of the cup and potentially fatigue failure. Variations in component positioning can lead to dynamic separation and edge loading [1]. The aim of this study was firstly to investigate the effects of translational and rotational positioning on the dynamic separation and severity of edge loading, and secondly to determine the wear rates of metal-on-polyethylene bearings under the more severe separation and edge loading conditions. Materials and Methods. A hip joint simulator (ProSim EM13, Simulation Solutions, UK) was set up with 36mm diameter metal-on-polyethylene hip replacements (Marathon™, DePuy Synthes Joint Reconstruction, Leeds, UK). This study was in two parts. I) A biomechanical test was carried out at 45° (n=3) and 65° (n=3) cup inclination angles with 1, 2, 3 and 4 (mm) medial-lateral translational mismatch between the head and cup centres. The severity of edge loading was calculated from the area under the axial force and medial-lateral force outputs during the time of separation when the load was acting on the edge of the cup [2]. II) For two conditions (two million cycles), the head and cup were concentric for cups inclined equivalent clinically to 45° (n=3) and 65° (n=3). For two further conditions (three million cycles), 4mm medial-lateral translational mismatch between centres was applied for cups inclined equivalent clinically to 45° (n=6) and 65° (n=6). Volumetric wear measurements were undertaken at one million cycle intervals. The lubricant was diluted new-born calf serum (25% v/v). Plastic deformation and wear were determined using a coordinate measurement machine. Mean values were calculated with 95% confidence limits. Statistical analysis was carried out using ANOVA and a t-test with significance levels taken at p<0.05. Results. The largest dynamic separation measured was at a 65° cup inclination angle with 4mm translational mismatch (Figure 1). At 1mm and 2mm of translational mismatch there was insignificant or no edge loading due to dynamic separation. The most severe edge loading conditions occurred at 4mm of translational mismatch at 45° and 65° inclination angles (p<0.01, Figure 2). The wear rates under standard concentric conditions were 12.9±3.8 and 15.4±5.0 mm. 3. /million cycles for cups inclined at 45° and 65° respectively. Higher wear rates were observed under 4mm of translational mismatch at 45° (21.5±5.5mm. 3. /million cycles, p<0.01) and 65° (23.0±5.7mm. 3. /million cycles, p<0.01) cup inclination angles. The mean maximum penetration depth at the edge of the cup at three million cycles was 0.10±0.05mm and 0.28±0.04mm at 45° and 65° cup inclination angles respectively (p<0.01), indicating substantial plastic deformation due to edge loading (Figure 3). Conclusion. Surgical positioning is important for long term clinical success of hip joint implants. A method has been developed to study the effects of rotational and translational positioning of metal-on-polyethylene bearings. Severe edge loading increased the wear and deformation of polyethylene liners at the rim. Minimising the occurrence and severity of edge loading and reducing the dynamic separation in vivo may reduce revision rates associated with polyethylene bearings

Introduction. In order to reduce polyethylene wear and incidence of osteolysis, and improve the long-term durability of total hip arthroplasty (THA), highly cross-linked polyethylene was introduced for clinical use in substitution for conventional polyethylene. We performed 35 cementless THAs between December 2000 and February 2002, and cross-linked polyethylene was used in these surgeries. The purpose of this study is to research linear wear rate of these hips, and to find the risk factor for high wear rate. Patients and Methods. 32 hips (26 patients) among the 35 could be evaluated at more than 10 years postoperatively. One hip required reoperations due to infection at 8 years postoperatively, and two were lost to followup in less than 10 years. There were 2 males and 24 females, and the observation period was 11.4 years in average (range 10?13 years). The age at the time of operation was 49.4 years in average (range 24?67 years), and body mass index was 22.4 in average (15?34). We used AHFIX total hip prostheses (KYOCERA Medical Corporation) for both acetabular and femoral replacement, and 22 mm Zirconia head was used in all cases. The median cup diameter was 46 mm (range 42?50). Osteolysis and loosening of the implant was evaluated on the anteroposterior radiograph of the hip. Using software for wear measurement (Hip Analysis Suite), linear wear rate and cup inclination angle were measured. Correlation between linear wear rate and age, BMI, cup inclination angle, and cup diameter was investigated using correlation coefficient. Results. Osteolysis and loosening were not found in any cases on the final radiograph. Cup inclination angle was 44.7±4.6 degree, and linear wear rate was 0.034±0.019 mm/y in average. No statistical correlation was found between linear wear rate and age, BMI, cup inclination angle, and cup diameter. Discussion. Long-term clinical performance of cross-linked polyethylene is rarely reported, although it is considered to have excellent wear resistance, and reduces the risk of osteolysis and loosening. Linear wear rate in this series was relatively low, and osteolysis and loosening were not observed. It was demonstrated that cross-linked polyethylene had excellent wear resistance when used in combination with 22 mm zirconia head, and observed for more than 10 years

Introduction and Aims. In order to improve the longevity and design of an implant, a wide range of pre-clinical testing conditions should be considered including variations in surgical delivery, and patients' anatomy and biomechanics. The aim of this research study was to determine the effect of the acetabular cup inclination angle with different levels of joint centre mismatch on the magnitude of dynamic microseparation, occurrence and severity of edge loading and the resultant wear rates in a hip joint simulator. Methods. The six-station Leeds Mark II Anatomical Physiological Hip Joint Simulator and 36mm diameter ceramic-on-ceramic bearings (BIOLOX® delta) were used in this study. A standard gait cycle, with a twin-peak loading (2.5kN peak load and approximately 70N swing phase load), extension/flexion 15°/+30° and internal/external ±10° rotations, was applied. Translational mismatch in the medial-lateral axis between the centres of rotation of the head and the cup were considered. In this study, mismatches of 2, 3 and 4 (mm) were applied. Two acetabular cup inclination angles were investigated; equivalent to 45° and 65° in-vivo. These resulted in a total of six conditions [Figure 1] with n=6 for each condition. Three million cycles were completed under each condition. The lubricant used was 25% (v/v) new-born calf serum supplemented with 0.03% (w/v) sodium azide to retard bacterial growth. The wear of the ceramic bearings were determined using a microbalance (XP205, Mettler Toledo, UK) and a coordinate measuring machine (Legex 322, Mitutoyo, UK). The stripe wear was analysed using RedLux software. The dynamic microseparation displacement was measured using a linear variable differential transformer. Mean wear rates and 95% confidence limits were determined and statistical analysis (one way ANOVA) completed with significance taken at p<0.05. Results Increasing the medial-lateral joint centre mismatch from 2 to 3 to 4mm resulted in an increased dynamic microseparation [Figure 2]. A similar trend was observed for the wear. A higher level of medial-lateral mismatch increased the wear rate under both 45° and 65° cup inclination angle conditions [Figure 3]. The mean wear rates obtained under 65° were significantly higher compared to those obtained under the 45° cup inclination angle conditions for a given medial-lateral mismatch in the joint centre (p=0.02 for 2mm mismatch, p=0.02 for 3 mm mismatch, and p<0.01 for 4mm mismatch). Conclusions. The condition with the acetabular cups positioned at an inclination of 45° exhibited greater resistance to dynamic microseparation for any given medial-lateral mismatch in the centres of rotation. Higher wear rates correlated with higher levels of dynamic microseparation. These results highlight how different conditions can alter the severity of edge loading, and highlight the necessity of understanding how the surgical positioning can affect the occurrence of edge loading and wear. Future studies will look into the other factors which can influence the microseparation conditions such as joint laxity, swing phase load and version angles

Introduction. The safe zone of the acetabular cup for THA was discussed based on the AP X-ray films of hip joints. A supine position is still used to determine the cup position for CAOS such as navigation systems. There were few data about the implant positions after THA in standing positions. The EOS X-Ray Imaging Acquisition System (EOS system) (EOS imaging Inc, Paris, France) allows image acquisition with the patients in a standing or sitting position. We can obtain AP and lateral X-ray images with high-quality resolution and low dose radiation exposure. Recently, we have obtained the EOS system for the first time in Japan. We investigated 3D accuracy of the EOS system for implant measurements after THA. Patients and Methods. We measured the implant angles of the 68 patients (59 females and 9 males, average age: 61y.o.) who underwent THA using the EOS system. The cup inclination and anteversion were measured in the anterior pelvic plane (APP) coordinate. The femoral stem antetorsion was defined as angles between the stem neck axis and the posterior condylar axis. These data were compared with the implant angles of the same patients measured by the post-operative CT scan images and the 3D image analysis using the ZedHip software (LEXI, Japan). Results. The cup inclinations (average ±SE) measured by the EOS system and the CT scan were 40.6 ± 0.64° and 42.9 ± 0.53°, respectively. The cup anteversions were 22.9 ± 1.3° and 22.8 ± 1.0°, respectively. The stem antetorsions were 28.9 ± 1.3° and 29.8 ± 1.6°, respectively. The differences (average ± SE) between the EOS system and the CT scan in the cup inclination, the cup anteversion, and the stem antetorsion were −2.3 ± 0.38°, −0.09 ± 0.82°, and −0.90 ± 0.91°, respectively. There were strong correlations in measurement values between the EOS system and the CT scan (the Spearman's correlation coefficients of the cup inclination, the cup anteversion, and the stem antetorsion were 0.6521 [p<0.001], 0.7154 [p<0.001], and 0.8645 [p<0.001], respectively). Discussion. The EOS system provides acceptable clinical accuracies in measuring acetabular cup and femoral stem angles after THA. The accuracy of the cup angles was accorded with that of the basic experimental data using a dry pelvis. Our data also demonstrated clinically acceptable accuracy in the measurement of stem antetorsion. This system can provide accurate snap shots of variable postures with high resolution. Using the EOS system, we may establish real optimum positions of THA implants by measuring the patients after THA in several postures including standing, squatting or sitting positions which required for Japanese ADL

Iliopsoas tendonitis after total hip arthroplasty (THA) can be a considerable cause of pain and patient dissatisfaction. The optimal cup position to avoid iliopsoas tendonitis has not been clearly established. Implant designs have also been developed with an anterior recess to avoid iliopsoas impingement. The purpose of this cadaveric study was to determine the effect of cup position and implant design on iliopsoas impingement. Bilateral THA was performed on three fresh frozen cadavers using oversized (jumbo) offset head center revision acetabular cups with an anterior recess (60, 62 and 66 mm diameter) and tapered wedge primary stems through a posterior approach. A 2mm diameter flexible stainless steel cable was inserted into the psoas tendon sheath between the muscle and the surrounding membrane to identify the location of the psoas muscle radiographically. CT scans of each cadaver were imported in an imaging software. The acetabular shells, cables as well as pelvis were segmented to create separate solid models of each. The offset head center shell was virtually replaced with an equivalent diameter hemispherical shell by overlaying the outer shell surfaces of both designs and keeping the faces of shells parallel. The shortest distance between each shell and cable was measured. To determine the influence of cup inclination and anteversion on psoas impingement, we virtually varied the inclination (30°/40°/50°) and anteversion (10°/20°/30°) angles for both shell designs. The CT analysis revealed that the original orientation (inclination/anteversion) of the shells implanted in 3 cadavers were as follows: Left1: 44.7°/23.3°, Right1: 41.7°/33.8°, Left2: 40/17, Right2: 31.7/23.5, Left3: 33/2908, Right3: 46.7/6.3. For the offset center shells, the shell to cable distance in all the above cases were positive indicating that there was clearance between the shells and psoas. For the hemispherical shells, in 3 out of 6 cases, the distance was negative indicating impingement of psoas. With the virtual implantation of both shell designs at orientations 40°/10°, 40°/20°, 40°/30° we found that greater anteversion helped decrease psoas impingement in both shell designs. When we analyzed the influence of inclination angle on psoas impingement by comparing wire distances for three orientations (30°/20°, 40°/20°, 50°/20°), we found that the effect was less pronounced. Further analysis comparing the offset head center shell to the conventional hemispherical shell revealed that the offset design was favored (greater clearance between the shell and the wire) in 17 out of 18 cases when the effect of anteversion was considered and in 15 out of 18 cases when the effect of inclinations was considered. Our results indicate that psoas impingement is related to both cup position and implant geometry. For an oversized jumbo cup, psoas impingement is reduced by greater anteversion while cup inclination has little effect. An offset head center cup with an anterior recess was effective in reducing psoas impingement in comparison to a conventional hemispherical geometry. In conclusion, adequate anteversion is important to avoid psoas impingement with jumbo acetabular shells and an implant with an anterior recess may further mitigate the risk of psoas impingement

Acetabular cup positioning remains a real challenge and component malpositioning after total hip arthroplasty (THA) can lead to increased rates of dislocation and wear. It is a common cause for revision THA. A novel 3D imageless mini-optical navigation system was used during THA to provide accurate, intraoperative, real-time, and non-fluoroscopic data including component positioning to the surgeon. This retrospective comparative single surgeon and single approach study examined acetabular component positioning between traditional mini-posterolateral THA and mini-posterolateral THA using the 3D mini-optical navigation system. A retrospective chart review was conducted of 157 consecutive (78 3D mini-optical navigation and 79 traditional non-navigation methods) THAs performed by the senior author using a mini-posterolateral approach at an ambulatory surgery center and hospital setting. Two independent reviewers analyzed postoperative radiographs in a standardized fashion to measure acetabular component positioning. Demographic, clinical, surgical, and radiographic data were analyzed. These groups were found to have no statistical difference in age, gender, and BMI (Table I). There was no difference between groups in acetabular components in the Lewinnek safe zone, 31.2% vs 26.6% (p = 0.53). Cup anteversion within the safe zone did not differ, 35.1% vs 40.5% (p = 0.48); while cup inclination within the safe zone differed, with more in the navigation group, 77.9% vs 51.9% (p < 0.01). Change in leg length was significantly different with the navigation group's leg length at 1.9 ± 6.3, less than the traditional at 5.4 ± 7.0 (p < 0.01). There was no difference in mean change in offset between groups (4.5 ± 5.9 vs 6.2 ± 7.9, p = 0.12); navigation, traditional) (Table II). The 3D mini-optical navigation group did have significantly longer operative time (98.4 ± 17.5 vs 89.3 ± 15.5 p < 0.01). Use of the novel 3D Mini-optical Navigation System significantly improved cup inclination compared to traditional methods while increasing operative time. Introduction and Aims. There are many surgical, implant design and patient factors that should be considered in preclinical testing of hip replacement which are not being considered in current standards. The aim of this study was to develop a preclinical testing method that consider surgical positioning, implant design and patient factors and predict the occurrence and severity of edge loading under the combination of such conditions. Then, assess the safety and reliability of the implant by predicting the wear, deformation and damage of the implant bearings under worst case conditions. Methods. Ceramic-on-ceramic (CoC, 36mm, BIOLOX. ®. delta, Pinnacle. ®. , DePuy Synthes, UK) and metal-on polyethylene (MoP, 36mm, Marathon®, Pinnacle. ®. , DePuy Synthes, UK) bearings were used for this study on multi-station multi-axis hip joint simulators. Two factors were varied, cup inclination angles (45° and 65°) and translational mismatch between the femoral head and acetabular cup (0, 2, 3 and 4 (mm)). Under each condition for both CoC and MoP bearings, three million cycles of gait cycle testing were completed with wear, deformation and/or damage measurements completed at one million cycle intervals. Other outputs of the study were the level of dynamic separation between the femoral head and acetabular cup during gait, the maximum force at the rim during edge loading when the head was sliding back to the cup confinement. Means and 95% confidence limits were determined and statistical analysis were done using one way ANOVA with significance taken at p<0.05. Results. As the level of mismatch and the cup inclination angle increased, the magnitude of dynamic separation and the force at the rim increased. The level of dynamic separation and the force on the rim correlated with the wear of CoC bearings (R= 0.96). For polyethylene, steeper inclination angle did not significantly increase the wear (p>0.05) however, edge loading under 4mm translational mismatch and steep cup inclination angle did (p<0.01). The combined wear and deformation of the polyethylene liners at the rim increased under larger levels of dynamic separation. Conclusions. The magnitude of dynamic separation and force at the rim were predictive of the severity of edge loading. These parameters can be measured using short term testing (500 cycles). This will determine the effect of variations in surgical positioning, implant design and patient factors on the occurrence and severity of edge loading. Then, the wear, deformation and/or damage on hip replacement bearings can be determined using longer term simulator testing under selected conditions. The short term tests do not only help identify worst case scenarios but may identify the boundary of surgical position under which the implants performance may be considered acceptable.
