III Oxford Medial Partial Knee is used to treat anteromedial osteoarthritis
(AMOA) in patients with an intact anterior cruciate ligament. This
unicompartmental knee arthroplasty (UKA) is relatively new in the
United States, and therefore long-term American results are lacking. This is a single surgeon, retrospective study based on prospectively
collected data, analysing a consecutive series of primary UKAs using
the Phase III mobile-bearing Oxford Knee and Phase III instrumentation. Between July 2004 and December 2006, the senior author (RHE)
carried out a medial UKA in 173 patients (213 knees) for anteromedial
osteoarthritis or avascular necrosis (AVN). A total of 95 patients were men and 78 were women. Their mean
age at surgery was 67 years (38 to 89) and mean body mass index
29.87 kg/m2 (17 to 62). The mean follow-up was ten years (4 to 11).Aims
Patients and Methods
There is a paucity of mid-term data on modular dual-mobility (MDM) constructs versus large (≥40 mm) femoral heads (LFH) in revision total hip arthroplasties (THAs). The purpose of this study was to update our prior series at 10 years, with specific emphasis on survivorships free of re-revision for dislocation, any re-revision, and dislocation. We identified 300 revision THAs performed at a single tertiary care academic institution from 2011 to 2014. Aseptic loosening of the acetabular component (n=65), dislocation (n=59), and reimplantation as part of a two-stage exchange protocol (n=57) were the most common reasons for index revision. Angular proximal femoral deformities increase the technical complexity of primary total hip arthroplasties (THAs). The goals were to determine the long-term implant survivorship, risk factors, complications, and clinical outcomes of contemporary primary THAs in this difficult cohort. Our institutional total joint registry was used to identify 119 primary THAs performed in 109 patients with an angular proximal femoral deformity between 1997 and 2017. The deformity was related to previous femoral osteotomy in 85%, and developmental or metabolic disorders in 15%. 53% had a predominantly varus angular deformity. The mean age was 44 years, mean BMI was 29 kg/m. 2. , and 59% were female. An uncemented metaphyseal fixation stem was used in 30%, an uncemented diaphyseal fixation stem in 28%, an uncemented modular body stem with metaphyseal fixation sleeve in 24%, and a cemented stem in 18%. Introduction. Many surgeons are reluctant to use a constrained liner at the time of acetabular component revision given concerns this might result in early acetabular component loosening. We hypothesized that with appropriate initial implant stabilization of highly porous acetabular components with supplemental screw fixation, constrained liners could be safely used at the time of acetabular revision. Methods. We retrospectively identified 148 revision total hip arthroplasties (THAs) where a constrained liner of one design was cemented into a newly placed highly porous acetabular component fixed with supplemental screws (mean 5 screws). Mean age at revision THA was 69 years, with 68% being female. The most common indications for revision were two-stage re-implantation (33%), recurrent dislocation (30%), and aseptic loosening (22% acetabular; 9% acetabular/femoral component). Mean follow-up was 8 years. Results. There were no failures at the bone-implant interface, and there were no revisions for aseptic loosening of the acetabular component. Introduction. Septic arthritis of the native knee often results in irreversible joint damage leading to the need for a total knee arthroplasty (TKA). This study examines the mid-term risk of periprosthetic joint infection (PJI), aseptic revision, any revision, and any reoperation in primary TKAs after septic arthritis of the native knee compared to a control cohort of primary TKAs performed for osteoarthritis (OA). Methods. We retrospectively identified 215 primary TKAs performed between 1971 and 2016 at a single institution following septic arthritis of the native knee. Eighty-two percent (177 cases) were treated in a single setting, whereas a two-stage exchange arthroplasty protocol was utilized in 18% (39 cases) for ongoing or suspected active native knee septic arthritis. Each case was matched 1:1 based on age, sex, body mass index (BMI), and surgical year to a primary TKA for OA. Mean age and BMI were 63 years and 30 kg/m2, respectively. Mean follow-up was 9 years. Results. Survivorship free from any infection (inclusive of PJI and wound infections) at 10 years was 87% in the septic arthritis cohort and 98% for the OA cohort (HR=6.5, p<0.01). Survivorship free of PJI at 10 years was 90% in the septic arthritis and 99% in the OA group (HR=6; p<0.01). There was no difference in the rate of infection when TKA occurred within 5 years of a septic arthritis diagnosis compared to a diagnosis that occurred >5 years from the TKA. The survivorship free of aseptic revision at 10 years was 83% for the septic arthritis cohort and 93% for the OA cohort (HR=2.5, p<0.01). When combining the above survivorships free of aseptic and septic revisions, the survivorship free of any revision at 10 years was 78% in the septic arthritis cohort and 91% in the OA cohort (HR=3, p<0.01). Background and aim. Since the market withdrawal of the ASR hip resurfacing in August 2010 because of a higher than expected revision rate as reported in the Australian Joint Replacement Registry (AOAJRR), metal-on-metal hip resurfacing arthroplasty (MoMHRA) has become a controversial procedure for hip replacement. Failures related to destructive adverse local tissue reactions to metal wear debris have further discredited MoMHRA. Longer term series from experienced resurfacing specialists however, demonstrated good outcomes with excellent 10-to-15-year survivorship in young and active men. These results have recently been confirmed for some MoMHRA designs in the AOAJRR. Besides, all hip replacement registries report significantly worse survivorship of total hip arthroplasty (THA) in patients under 50 compared to older ages. The aim of this study was to review MoMHRA survivorship from the national registries reporting on hip resurfacing and determine the risk factors for revision in the different registries. Methods. Introduction. The BIRMINGHAM HIP. ◊. Resurfacing is a metal-on-metal (MOM) hip implant system approved by the US FDA in 2006. The approval required a multicenter, prospective, post-approval study (PAS). Our purpose is to report the current minimum 10-year results. Methods. 253 patients (280 hips) had surgery between October 2006 and December 2009 at one of 5 sites. We report revisions, survivorship, EQ-5D, Harris Hip Score (HHS), radiographic findings, and metal levels including cobalt (Co) and chromium (Cr). The mean age at surgery was 51 years, 74% male, BMI 28, osteoarthritis 95%. 243 (87%) of hips have known outcome or 10-year minimum follow-up (fup). Prior to 10 years, 5 patients died, 20 hips were revised, and 37 hips did not complete 10-year fup. Results. The
Background: Hydroxyapatite (HA) coating is widely used for total hip arthroplasty as it has been suggested to improve implant ingrowth and long-term stability. However, the evidence behind the use of HA in femoral stems is ambiguous. Methods: We investigated a non-cemented, tapered titanium femoral stem that was available either with or without HA coating. This stem had been used in 3,116 total hip arthroplasties (THAs) in 2,608 patients registered in the Swedish Hip Arthroplasty Register (1992–2007). Kaplan-Meier survival analysis and a Cox regression model including type of coating, age, sex, primary diagnosis, and the type of cup fixation were used to calculate adjusted risk ratios (RR) of the risk for revision for various reasons. The clinical success of THA is accepted, however wide variation in implant usage and outcomes is reported across the world. The Getting It Right First Time initiative in the UK has recommended increased use of cemented constructs in patients over 70. A clear clinical directive has been proposed aiming to reduce revision rates, improve outcomes and increase cost effectiveness. This study compares the outcomes of cemented, hybrid and cementless THA from a high-volume single center at long-term follow-up. A prospective database identified 1,699 cemented, 5,782 hybrid and 2,631 cementless THAs. Mean follow-up was 8.1 years, 5.3 years and 6.3 years respectively. Primary endpoint was revision for any reason. 47 (1.79%) cemented, 93 (1.60%) hybrid and 39 (1.48%) cementless THAs underwent revision. The difference in overall revision rate was statistically significant (P=0.002). Mean time to revision was 4.5 years in the cemented group, 2.1 years in the hybrid group and 2.6 years in the cementless group. 1 year survivorship was 99.2% in the cemented group, 99.0% in the hybrid group and 98.9% in the cementless group. 3 year survivorship was 98.6% in the cemented group, 98.8% in the hybrid group and 98.9% in the cementless group. Introduction. Patients undergoing primary total hip arthroplasty (THA) following pelvic radiation have historically had poor survivorship free of aseptic acetabular component loosening. However, several series have reported improved results with tantalum acetabular components. The purpose of this study was to assess implant survivorship, radiographic results, and clinical outcomes of contemporary, non-tantalum, porous acetabular components in the setting of prior pelvic radiation. Methods. We retrospectively reviewed 33 patients (38 hips) with prior therapeutic pelvic radiation between 2006 and 2016 who underwent primary THA. The mean overall pelvic radiation dose was 6300 cGy with a mean latency period to THA of 5 years. The most common acetabular component was Pinnacle (Depuy-Synthes) in 76%, followed by Trident (Stryker) in 8%, Tritanium (Stryker) in 8%, Trilogy (Zimmer-Biomet) in 5%, and G7 (Zimmer-Biomet) in 3%. Introduction. Both mobile bearing and fixed bearing unicompartmental knee arthroplasty (UKA) have demonstrated clinical success. However, much debate persists about the superiority of a single design. Currently most clinical data is based on high volume centers data, however to reduce bias, we undertook a through review of retrospective national joint registries. In this study, we aim to investigate UKA implant utilization and survivorship between 2000 and 2018. Methods. Ten annual joint registry reports of various nations were reviewed. Due to the variable statistical methods of reporting implant use and survivorship we focused on three registries: Australia (AOANJRR), New Zealand (NZJR), United Kingdom (NJR) for uniformity. We evaluated UKA usage, survivorship, utilization and revision rates for each implant. Implant survivorship was reported in the registries and was compared within nations due to variation in statistical reporting. Results. UKAs accounted for 7.24–10.3% of all primary knee surgeries. The most popular implants were, in order of popularity, the Oxford (ZimmerBiomet, Warsaw IN), ZUK (Smith & Nephew, Memphis TN / LimaCorporate, Udine, Italy), Sigma HP (DePuy, Warsaw IN), Miller-Galante (ZimmerBiomet Warsaw IN), and Preservation (DePuy, Warsaw IN) models. UKA revision rates varied by nation and ranged from 8.22%–12.8%. The Sigma HP and ZUK designs had the lowest total percent revised in the registries-Australian (3.6% and 5.1%, respectively) and New Zealand (2.5% and 1.96%, respectively)-and demonstrated among the highest
The aim of this study was to determine the medium term survivorship and function of the cemented Exeter Universal Hip Replacement when used in younger patients. Since 1988 The Exeter Hip Research Unit has prospectively gathered data on all patients who have had total hip replacements at the Princess Elizabeth Orthopaedic Hospital. There were 88 Exeter Universal total hip replacements (THR) in 71 patients who were 50 years or younger at the time of surgery and whose surgery was performed at least 10 years before. 25 surgeons performed the surgery. Mean age at surgery was 43 years (range 24 to 50 years. ) 5 patients who had 7 THRs had died leaving 81 THRs for review. Patients were reviewed in clinic at an average of 11. 4 years (10 – 13 years). No patient was lost to follow up. At review, 8 hips had been revised. 5 cases were for loose cemented metal backed acetabular prosthesis. Two femoral components were revised for infection and one for aseptic loosening. Radiographs showed that a further 10 (13%) acetabular prosthesis were loose and that 3 femurs showed significant osteolysis. Overall
Tapered fluted grit-blasted modular stems have now become established as a successful method of femoral revision. The success of these stems is predicated on obtaining axial stability by milling the femur to a cone and then inserting the tapered prosthesis into that cone. Torsional stability is gained by flutes that cut into the diaphysis. By having modular proximal segments of different lengths, the leg length, offset, and anteversion can be adjusted after the distal stem is fixed. This maximises the chance for the stem to be driven into the canal to whatever level provides maximum stem stability. Modular fluted tapered stems have the potential benefits of being made of titanium and hence being both bone friendly and also having a modulus of elasticity closer to that of bone. They have a well-established high rate of fixation. Drawbacks include the risk of fracture of modular junctions and tapers, and difficulty of extraction. The indications for the use of these implants vary among surgeons, but the implants are suitable for use in a wide variety of bone loss categories. Non-modular fluted tapered stems also can gain excellent fixation, but are less versatile and in most practices are used for selected simpler revisions. The Exeter V40 femoral stem is the most implanted stem in the NJR for primary THA. In 2004, the 44/00/125 stem was released for use in “cement-in-cement” revision cases. It has however been used ‘off-label’ as a primary stem when, for example, patient anatomy requires a smaller stem with a 44mm offset. We aimed to investigate survival of this stem in comparison to others in the range when used in primary THAs recorded in the NJR. Analyses were performed using a dataset based on that used for the 2020 NJR annual report. Our exposure was the stem; the outcome was all-cause construct revision. Crude analyses were performed using Kaplan-Meier and adjusted using Cox models. The 44/00/125 stem was directly compared to other stems in the Exeter range. We analysed 330,732 primary THAs using the Exeter V40 stem comprising 34.5% of the 958,869 THAs with complete information from the start of the NJR to 31 December 2018. The 44/00/125 stem was implanted in 2,158 primary THAs with 67.5% in female patients and a mean age of 67.8. The 10-year revision estimate for the 44/00/125 stem was 4.9% (95%CI 3.6, 6.8) and in constructs using an Exeter V40 stem was 2.8% (95%CI 2.7, 2.8). Controlling for age, sex and ASA demonstrated an increased overall hazard of revision for constructs using the 44/00/125 stem compared to constructs using other Exeter V40 femoral stems (HR 1.8 (95%CI 1.4, 2.3)). In revision total hip arthroplasty (THA), acetabular reconstruction while dealing with severe bone loss is a challenge. The porous tantalum revision acetabular shells have been in use for the past decade. Several reports have documented successful use at early to mid-term follow up. There is, however, very little literature around the long-term survival and quality of life outcome with the use of these shells. We reviewed the results of 46 acetabular revisions with Paprosky 2 and 3 acetabular bone defects reconstructed with a hemispheric, tantalum acetabular shell and multiple supplementary screws. There were 31 females. Average age at revision was 64 years (range 23–85 years). The mean and median follow up was 11 years (range 10–12 years, SD 1). Morselised femoral allograft was used in 34 hips to fill contained cavitary defectes. Bulk femoral allografting was performed in 2 hips. Introduction. We sought to determine the
Purpose. The success of total knee replacement (TKR) surgery can be attributed to improvements in TKR design, instrumentation, and surgical technique. Over a decade ago oxidized zirconium (OxZr) femoral components were introduced as an alternative bearing surface to cobalt-chromium (CoCr), based on strong in-vitro evidence, to improve the longevity of TKR implants. Early reports have demonstrated the clinical success of this material however no long-term comparative studies have demonstrated the superiority of OxZr implants compared to a more traditional CoCr implant. This study aims to compare long-term survivorship and outcomes in OxZr and CoCr femoral components in a single total knee design. Methods. We reviewed our institutional database to identify all patients whom underwent a TKA with a posterior stabilized OxZr femoral component with a minimum of 10 years of follow-up. These were then matched to patients whom underwent a TKA with the identical design posterior stabilized CoCr femoral component during the same time period by gender, age and BMI. All patients had their patella resurfaced. All patients were prospectively evaluated preoperatively and postoperatively at 6 weeks, 3 months, 12 months, 2 years and every 1 to 2 years thereafter. Prospectively collected clinical outcome measures included, Western Ontario and McMaster Universities osteoarthritis index (WOMAC), Short-Form 12 (SF-12) and Knee Society clinical rating scores (KSCRS). Charts and radiographs were reviewed to determine the revision rates and survivorship (both all cause and aseptic) at 10 years allowing comparison between the two cohorts. Paired analysis was performed to determine if differences existed in patient reported outcomes. Results. There were 194 OxZr TKAs identified and matched to 194 CoCr TKAs. There was no difference in average age (OxZr, 54.2 years; CoCr, 54.4 years), Gender (OxZr, 61.9% female; CoCr, 61.9% female) and average body mass index (OxZr, 35.9 kg/m2; CoCr, 36.4 kg/m2) between the two cohorts. There were no significant differences preoperatively in any of the outcome scores between the two groups (WOMAC (p=0.361), SF-12 (p=0.771) and KSCRS (p=0.161)). Both the SF12 (p=0.787) and WOMAC (p=0.454) were similar between the two groups, however the OxZr TKA cohort had superior KSCRS compared to the CoCr TKA cohort at a minimum of 10 years (173.5 vs. 159.1, p=0.002). Introduction & Objective. Labral refixation has established as a standard in open or arthroscopic treatment for femoroacetabular impingement (FAI). The rationale for this refixation is to maintain the important suction seal in the hip. To date, only few short-term results are available which indicate a superior result in FAI hips with labral refixation compared to labral resection.
