Aims

When performing revision total hip arthroplasty using diaphyseal-engaging titanium tapered stems (TTS), the recommended 3 to 4 cm of stem-cortical diaphyseal contact may not be available. In challenging cases such as these with only 2 cm of contact, can sufficient axial stability be achieved and what is the benefit of a prophylactic cable? This study sought to determine, first, whether a prophylactic cable allows for sufficient axial stability when the contact length is 2 cm, and second, if differing TTS taper angles (2° vs 3.5°) impact these results.

Methods

A biomechanical matched-pair cadaveric study was designed using six matched pairs of human fresh cadaveric femora prepared so that 2 cm of diaphyseal bone engaged with 2° (right femora) or 3.5° (left femora) TTS. Before impaction, three matched pairs received a single 100 lb-tensioned prophylactic beaded cable; the remaining three matched pairs received no cable adjuncts. Specimens underwent stepwise axial loading to 2600 N or until failure, defined as stem subsidence > 5 mm.

A spine compression fracture is a very common form of fracture in elderly with osteoporosis. Injection of polymethyl methacrylate (PMMA) to fracture sites is a minimally invasive surgical treatment, but PMMA has considerable clinical risks. We develop a novel type thermoplastic injectable bone substitute contains the proprietary composites of synthetic ceramic bone substitute and absorbable thermoplastic polymer.

We used thermoplastic biocompatible polymers Polycaproactone (PCL) to encapsulate calcium-based bone substitutes hydroxyapatite (Ca10(PO4)6(OH)2, HA) and tricalcium phosphate (TCP) to form a biodegradable injectable bone composite material. The space occupation ration PCL:HA/TCP is 1:9. After heating process, it can be injected to fracture site by specific instrument and then self-setting to immediate reinforce the vertebral body.

The thermoplastic injection bone substitute can obtain good injection properties after being heated by a heater at 90˚C for three minutes, and has good anti-washout property when injected into normal saline at 37˚C. After three minutes, solidification is achieved. Mechanical properties were assessed using the material compression test system and the mechanical support close to the vertebral spongy bone.

In vitro cytotoxicity MTT assay (3-(4,5-Dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide) was performed and no cell cytotoxicity was observed.

In vivo study with three New Zealand rabbits was performed, well bone growth into bone substitute was observed and can maintain good mechanical support after three months implantation.

The novel type thermoplastic injection bone substitute can achieve (a) adequate injectability and viscosity without the risk of cement leakage; (b) adequate mechanical strength for immediate reinforcement and prevent adjacent fracture; (c) adequate porosity for new bone ingrowth; (e) biodegradability. It could be developed as a new option for treating vertebral compression fractures.

Distal femur fractures (DFF) are common, especially in the elderly and high energy trauma patients. Lateral locked osteosynthesis constructs have been widely used, however non-union and implant failures are not uncommon. Recent literature advocates for the liberal use of supplemental medial plating to augment lateral locked constructs. However, there is a lack of proprietary medial plate options, with some authors supporting the use of repurposing expensive anatomic pre-contoured plates. The aim of this study was to investigate the feasibility of a readily available cost-effective medial implant option.

A retrospective analysis from January 2014 to June 2022 was performed on DFF (primary or revision) managed with supplemental medial plating with a Large Fragment Locking Compression Plate (LCP) T-Plate (~$240 AUD) via a medial sub-vastus approach. The T-plate was contoured and placed superior to the medial condyle. A combination of 4.5mm cortical, 5mm locking and/or 6.5mm cancellous screws were used, with oblique screw trajectories towards the distal lateral cortex of the lateral condyle. All extra-articular fractures and revision fixation cases were allowed to weight bear immediately. The primary outcome was union rate.

This technique was utilised on sixteen patients; 3 acute, 13 revisions; mean age 52 years (range 16-85), 81% male, 5 open fractures. The union rate was 100%, with a median time to union of 29 weeks (IQR 18-46). The mean follow-up was 15 months. There were two complications: a deep infection requiring two debridements and a prominent screw requiring removal. The mean range of motion was 1–108o.

Supplemental medial plating of DFF with a Large Fragment LCP T-Plate is a feasible, safe, and economical option for both acute fixation and revisions. Further validation on a larger scale is warranted, along with considerations to developing a specific implant in line with these principles.

Aims

Hospital case volume is shown to be associated with postoperative outcomes in various types of surgery. However, conflicting results of volume-outcome relationship have been reported in hip fracture surgery. This retrospective cohort study aimed to evaluate the association between hospital case volume and postoperative outcomes in patients who had hip fracture surgery. We hypothesized that higher case volume would be associated with lower risk of in-hospital and one-year mortality after hip fracture surgery.

Methods

Data for all patients who underwent surgery for hip fracture from January 2008 to December 2016 were extracted from the Korean National Healthcare Insurance Service database. According to mean annual case volume of surgery for hip fracture, hospitals were classified into very low (< 30 cases/year), low (30 to 50 cases/year), intermediate (50 to 100 cases/year), high (100 to 150 cases/year), or very high (> 150 cases/year) groups. The association between hospital case volume and in-hospital mortality or one-year mortality was assessed using the logistic regression model to adjust for age, sex, type of fracture, type of anaesthesia, transfusion, comorbidities, and year of surgery.

Aims

The purpose of this study was to identify the changes in untreated long head of the biceps brachii tendon (LHBT) after a rotator cuff tear and to evaluate the factors related to the changes.

Methods

A cohort of 162 patients who underwent isolated supraspinatus with the preservation of LHBT was enrolled and evaluated. The cross-sectional area (CSA) of the LHBT on MRI was measured in the bicipital groove, and preoperative to postoperative difference was calculated at least 12 months postoperatively. Second, postoperative changes in the LHBT including intratendinous signal change, rupture, dislocation, or superior labral lesions were evaluated with seeking of factors that were correlated with the changes or newly developed lesions after rotator cuff repair.

Aims

Commonly performed unicompartmental knee arthroplasty (UKA) is not designed for the lateral compartment. Additionally, the anatomical medial and lateral tibial plateaus have asymmetrical geometries, with a slightly dished medial plateau and a convex lateral plateau. Therefore, this study aims to investigate the native knee kinematics with respect to the tibial insert design corresponding to the lateral femoral component.

Methods

Subject-specific finite element models were developed with tibiofemoral (TF) and patellofemoral joints for one female and four male subjects. Three different TF conformity designs were applied. Flat, convex, and conforming tibial insert designs were applied to the identical femoral component. A deep knee bend was considered as the loading condition, and the kinematic preservation in the native knee was investigated.

Objectives

Unicompartmental knee arthroplasty (UKA) is an alternative to total knee arthroplasty with isolated medial or lateral compartment osteoarthritis. However, polyethylene wear can significantly reduce the lifespan of UKA. Different bearing designs and materials for UKA have been developed to change the rate of polyethylene wear. Therefore, the objective of this study is to investigate the effect of insert conformity and material on the predicted wear in mobile-bearing UKA using a previously developed computational wear method.

Methods

Two different designs were tested with the same femoral component under identical kinematic input: anatomy mimetic design (AMD) and conforming design inserts with different conformity levels. The insert materials were standard or crosslinked ultra-high-molecular-weight polyethylene (UHMWPE). We evaluated the contact pressure, contact area, wear rate, wear depth, and volumetric wear under gait cycle loading conditions.


Objectives

Unicompartmental knee arthroplasty (UKA) is an alternative to total knee arthroplasty for patients who require treatment of single-compartment osteoarthritis, especially for young patients. To satisfy this requirement, new patient-specific prosthetic designs have been introduced. The patient-specific UKA is designed on the basis of data from preoperative medical images. In general, knee implant design with increased conformity has been developed to provide lower contact stress and reduced wear on the tibial insert compared with flat knee designs. The different tibiofemoral conformity may provide designers the opportunity to address both wear and kinematic design goals simultaneously. The aim of this study was to evaluate wear prediction with respect to tibiofemoral conformity design in patient-specific UKA under gait loading conditions by using a previously validated computational wear method.

Methods

Three designs with different conformities were developed with the same femoral component: a flat design normally used in fixed-bearing UKA, a tibia plateau anatomy mimetic (AM) design, and an increased conforming design. We investigated the kinematics, contact stress, contact area, wear rate, and volumetric wear of the three different tibial insert designs.

Purpose

The purpose of this study is to compare the mid-term outcomes between rotating platform(RP) and posterior cruciate substitute(PS) bearings in LCS-total knee arthroplasty (TKA).

Methods

This is a prospective randomized matched-pairs case–control study of patients who underwent primary TKAs with a minimum 2 years follow-up. The inclusion criteria was degenerative arthritis, and exclusion criteria was arthritis other than degeneration, previous operations such as osteotomy, body mass index (BMI) over 30 kg/m2. The matching criteria were set as age, BMI, preoperative ROM, gender, and the Knee Society (KS) and Hospital for Special Surgery (HSS) scores. After 1:1 matching, 50 cases involved the LCS RP bearing system as group 1 and 50 cases involved LCS PS bearing system group as group 2. Clinical and radiographic outcomes were evaluated.

Introduction

The range of motion (ROM) obtained after total knee arthroplasty (TKA) is an important measurement to evaluate the postoperative outcomes impacting other measures such as postoperative function and satisfaction. Flexion contracture is a recognized complication of TKA, which reduces ROM or stability and is a source of morbidity for patients.

Objectives

The purpose of this study was to evaluate the influence of intra-operative soft tissue release on correction of flexion contracture in navigated TKA.

Introduction

Mechanically aligned total knee arthroplasty(TKA) relies on restoring the hip-knee-ankle angle of the limb to neutral or as close to a straight line as possible. This principle is based on studies that suggest limb and knee alignment is related long term survival and wear. For that cause, there has been recent attention concerning computer-assisted TKA and robot is also one of the most helpful instruments for restoring neutral alignment as known. But many reported data have shown that 20% to 25% of patients with mechanically aligned TKA are dissatisfied. Accordingly, kinematically aligned TKA was implemented as an alternative alignment strategy with the goal of reducing prevalence of unexplained pain, stiffness, and instability and improving the rate of recovery, kinematics, and contact forces. So, we want to report our extremely early experience of robot-assisted TKA planned by kinematic method.

Materials and Methods

This study evaluated the very short term results (6 weeks follow up) after robot-assisted TKA aligned kinematically. 50 knees in 36 patients, who could be followed up more than 6 weeks after surgery from December 2014 to January 2015, were evaluated prospectively. The diagnosis was primary osteoarthritis in all cases. The operation was performed with ROBODOC (ISS Inc., CA, USA) along with the ORTHODOC (ISS Inc., CA, USA) planning computer. The cutting plan was made by single radius femoral component concept, each femoral condyles shape-matched method along the transverse axis using multi-channel CT and MRI to place the implant along the patient's premorbid joint line. Radiographic measurements were made from long bone scanograms. Clinical outcomes and motion were measured preoperatively and 6 weeks postoperatively.

Purpose

A Trabecular Metal Modular Acetabular System (Zimmer, Warsaw, Indiana, USA) is a peripheral rim expansion (elliptical) cup, i.e. a non-hemispherical cup. Radiologically a non-hemispherical cup may be deferent from other conventional hemispherical cups. We reviewed radiological findings of a Trabecular Metal Modular Acetabular System chronologically.

Methods

Twenty six patients with osteoarthritis underwent primary total hip arthroplasty (THA) using a Trabecular Metal Modular Acetabular System from 2011 to April 2013. Twenty five patients (follow-up rate: 96.2%) 31 hips could be followed-up over a year were registered. In common, the diameter of every femoral head was 32 mm. We planned the acetabular cup inclination angle to be 45-degree, the cup coverage with host-bone (cup-CE angle) to be over 10-degree, and high hip center was allowed up to 20mm. In case of the cup-CE angle under 10-degree, an acetabular cup was placed medially using Dorr's medial protrusio technique. We established the medial protrusion angle indicating the degree of medial protrusion of an acetabular cup over the pelvic internal wall. The medial protrusion angle was defined by the center point of THA (C) and the 2 cross-points (X1, X2) which the outline of an acetabular cup crosses the Kohler's line (Figure 1). The cup anteversion angle was measured by the method of Lewinnek, and the cup fixation was evaluated according to the Tompkin's classification.

Although gradual bone transport may permit the restoration of large-diameter bones, complications are common owing to the long duration of external fixation. In order to reduce such complications, a new technique of bone transport involving the use of an external fixator and a locking plate was devised for segmental tibial bone defects.

A total of ten patients (nine men, one woman) with a mean age at operation of 40.4 years (16 to 64) underwent distraction osteogenesis with a locking plate to treat previously infected post-traumatic segmental tibial defects. The locking plate was fixed percutaneously to bridge proximal and distal segments, and was followed by external fixation. After docking, percutaneous screws were fixed at the transported segment through plate holes. At the same time, bone grafting was performed at the docking site with the external fixator removed.

The mean defect size was 5.9 cm (3.8 to 9.3) and mean external fixation index was 13.4 days/cm (11.8 to 19.5). In all cases, primary union of the docking site and distraction callus was achieved, with an excellent bony result. There was no recurrence of deep infection or osteomyelitis, and with the exception of one patient with a pre-existing peroneal nerve injury, all achieved an excellent or good functional result.

With short external fixation times and low complication rates, bone transport with a locking plate could be recommended for patients with segmental tibial defects.

Introduction

The success of total knee arthroplasty depends on many factors, including the preoperative condition of the patient, the design and materials of the components and surgical techniques. It is important to position the femoral and tibial components accurately and to balance the soft tissues. Malpositioning of the component can lead to failures due to aseptic loosening, instability, polyethylene wear and dislocation of the patella. In order to improve post-operative alignment, computer-aid systems have been developed for total knee arthroplasty. Many clinical and experimental studies of these systems have shown that the accuracy of implanted components can be improved in spite of the increase in costs and operating time. This may not, however, improve the outcome in the short-term. Restoration of the normal mechanical axis of the knee and balancing of the surrounding soft tissues have been shown to have an important bearing on the final outcome of knee replacement operations. In severely deformed knees, whether varus or valgus, these goals may be difficult to achieve. We compared the radiologic results of the mechanical axis and implant position of Total Knee Arthroplasty using a robot-assisted method with conventional manually implanted method in severe varus deformed knee.

Materials and Methods

A data set of 50 consecutive cases that were performed from April 2007 to December 2010 using the robot assisted TKA(Group A) were compared with a data set of 50 consecutive cases from the same period that were done using conventional manual TKA(Group B). All cases had a preoperative mechanical varus deformity >15° and one brand of implant was used on all cases. The diagnosis was primary osteoarthritis in all knees. The operations were performed by one-senior author with the same robot system, ROBODOC (ISS Inc., CA, USA) along with the ORTHODOC (ISS Inc., CA, USA) planning computer. (See Figure 1.) The radiological evaluations included mechanical axis, implant position (α,β,γ,δ angle) according to the system of American Knee Society.

Introduction

Since Smith-Peterson's glass mold arthroplasty in 1939, hip resurfacing arthroplasty was developed and introduced to orthopaedic surgery field but it had many problem like early loosening. Recently it is being popular for some indication as development of new implant design and manufacturing. There are still many suggested advantages of hip resurfacing arthroplasty. These include bone conservation, improved function as a consequence of retention of the femoral head and neck and more precise biomechanical restoration, decreased morbidity at the time of revision arthroplasty, reduced dislocation rates, normal femoral loading and reduced stress-shielding, simpler management of a degenerated hip with a deformity in the proximal femoral metaphysic, an improved outcome in the event of infection, and a reduced prevalence of thromboembolic phenomena as a consequence of not using instruments in the femur. But, there are limited or inconsistent data to support some of these claims regarding the benefits of hip resurfacing including the potential for a more natural feel because of the minimal disturbance of the proximal part of the femur resulting in a better and faster functional outcome. We evaluate the short term results of hip resurfacing arthroplasty using custom patient-specific tooling for prosthesis placement for better standardization.

Materials and Methods

40 cases, 36 patients(male:20, female:16) those of who were candidates of a Hip Resurfacing procedure, participated in the study. Mean follow up period was 2.5 years (8 months ∼3 years). A CT scan was performed on each patient and a 3D model was generated using the computer tomography dataset. From this model a bone-surface skin was extracted and this data set was used to create a personalized jig. Detailed analysis of the native bone structure was then used to preoperatively plan the appropriate size and position of the implant. A mean 7 degree corrective valgus angle was prescribed on all cases. Postoperative radiological datasets were superimposed onto preoperative plan position and offsets were measured. Operative times were recorded per step during the procedure. Surgeon comfort and ease of use was also noted.

This study evaluated the effect of limb lengthening on longitudinal growth in patients with achondroplasia. Growth of the lower extremity was assessed retrospectively by serial radiographs in 35 skeletally immature patients with achondroplasia who underwent bilateral limb lengthening (Group 1), and in 12 skeletally immature patients with achondroplasia who did not (Group 2). In Group 1, 23 patients underwent only tibial lengthening (Group 1a) and 12 patients underwent tibial and femoral lengthening sequentially (Group 1b).

The mean lengthening in the tibia was 9.2 cm (59.5%) in Group 1a, and 9.0 cm (58.2%) in the tibia and 10.2 cm (54.3%) in the femur in Group 1b. The mean follow-up was 9.3 years (8.6 to 10.3). The final mean total length of lower extremity in Group 1a was 526.6 mm (501.3 to 552.9) at the time of skeletal maturity and 610.1 mm (577.6 to 638.6) in Group 1b, compared with 457.0 mm (411.7 to 502.3) in Group 2. However, the mean actual length, representing the length solely grown from the physis without the length of distraction, showed that there was a significant disturbance of growth after limb lengthening. In Group 1a, a mean decrease of 22.4 mm (21.3 to 23.1) (4.9%) was observed in the actual limb length when compared with Group 2, and a greater mean decrease of 38.9 mm (37.2 to 40.8) (8.5%) was observed in Group 1b when compared with Group 2 at skeletal maturity.

In Group 1, the mean actual limb length was 16.5 mm (15.8 to 17.2) (3.6%) shorter in Group 1b when compared with Group 1a at the time of skeletal maturity. Premature physeal closure was seen mostly in the proximal tibia and the distal femur with relative preservation of proximal femur and distal tibia.

We suggest that significant disturbance of growth can occur after extensive limb lengthening in patients with achondroplasia, and therefore, this should be included in pre-operative counselling of these patients and their parents.

Lengthening of the humerus is now an established technique. We compared the complications of humeral lengthening with those of femoral lengthening and investigated whether or not the callus formation in the humerus proceeds at a higher rate than that in the femur. A total of 24 humeral and 24 femoral lengthenings were performed on 12 patients with achondroplasia. We measured the pixel value ratio (PVR) of the lengthened area on radiographs and each radiograph was analysed for the shape, type and density of the callus. The quality of life (QOL) of the patients after humeral lengthening was compared with that prior to surgery. The complication rate per segment of humerus and femur was 0.87% and 1.37%, respectively. In the humerus the PVR was significantly higher than that of the femur. Lower limbs were associated with an increased incidence of concave, lateral and central callus shapes. Humeral lengthening had a lower complication rate than lower-limb lengthening, and QOL increased significantly after humeral lengthening. Callus formation in the humerus during the distraction period proceeded at a significantly higher rate than that in the femur.

These findings indicate that humeral lengthening has an important role in the management of patients with achondroplasia.

We examined the differences in post-operative functional disability and patient satisfaction between 56 patients who underwent a lumbar fusion at three or more levels for degenerative disease (group I) and 69 patients, matched by age and gender, who had undergone a one or two level fusion (group II). Their mean age was 66 years (49 to 84) and the mean follow-up was 43 months (24 to 65).

The mean pre-operative Oswestry Disability Index (ODI) and visual analogue scale (VAS) for back and leg pain, and the mean post-operative VAS were similar in both groups (p >  0.05), but post-operatively the improvement in ODI was significantly less in group I (40.6%) than in group II (49.5%) (p < 0.001). Of the ten ODI items, patients in group I showed significant problems with lifting, sitting, standing, and travelling (p < 0.05). The most significant differences in the post-operative ODI were observed between patients who had undergone fusion at four or more levels and those who had undergone fusion at less than four levels (p = 0.005). The proportion of patients who were satisfied with their operations was similar in groups I and II (72.7% and 77.0%, respectively) (p = 0.668). The mean number of fused levels was associated with the post-operative ODI (r = 0.266, p = 0.003), but not with the post-operative VAS or satisfaction grade (p > 0.05). Post-operative functional disability was more severe in those with a long-level lumbar fusion, particularly at four or more levels, but patient satisfaction remained similar for those with both long- and short-level fusions.

The patellofemoral joint is an important source of symptoms in osteoarthritis of the knee. We have used a newly designed surgical model of patellar strengthening to induce osteoarthritis in BALB/c mice and to establish markers by investigating the relationship between osteoarthritis and synovial levels of matrix metalloproteinases (MMPs). Osteoarthritis was induced by using this microsurgical technique under direct vision without involving the cavity of the knee. Degeneration of cartilage was assessed by the Mankin score and synovial tissue was used to determine the mRNA expression levels of MMPs. Irrigation fluid from the knee was used to measure the concentrations of MMP-3 and MMP-9. Analysis of cartilage degeneration was correlated with the levels of expression of MMP.

After operation the patellofemoral joint showed evidence of mild osteoarthritis at eight weeks and further degenerative changes by 12 weeks. The level of synovial MMP-9 mRNA correlated with the Mankin score at eight weeks, but not at 12 weeks. The levels of MMP-2, MMP-3 and MMP-14 mRNA correlated with the Mankin score at 12 weeks. An increase in MMP-3 was observed from four weeks up to 16 weeks. MMP-9 was notably increased at eight weeks, but the concentration at 16 weeks had decreased to the level observed at four weeks.

Our observations suggest that MMP-2, MMP-3 and MMP-14 could be used as markers of the progression of osteoarthritic change.

The anterior pelvic plane has been introduced as a concept of the reference plane to image free navigation-assisted cup placement of total hip arthroplasty. With the neutral pelvis, the anteversion relative to the conventional coordinate system is equal to the that of relation to the anatomical coordinate system. This is the rationale of image free navigation system. But, currently, two major concerns about image-free navigation assisted total hip arthroplasty are tilting of anatomic coordinate system and the cutaneous palpation procedure. Therefore, it was the goal of this study to provide both the bone anterior pelvic plane (Bone_APP) and the overlying soft tissue plane (Soft_APP) simultaneously, and to find possible correlations of biometrical parameters and effect of ante-version were an additional motivation of this study. 23 Korean adult patients underwent image-free navigation-assisted total hip arthroplasty. The tilting of Bone_APP, soft tissue thickness on ASIS, pubis, and then tilting of Soft_APP, and anteversion of cup were measured with reconstructed CT and 3D workstation system.

The average age was 66.1 years, the average height was 162.5cm at a weight of 59.2 kg. The average body mass index was 22.3. And the average lumbar lordosis was measured as 30.4 degrees. The soft tissue on the level of the pubis was 17.6 mm thicker than that on the level of ASIS in average. In all cases, Soft_APP was positive, that is from 3.5 to 16.5 degrees of backward rotation. We also found a high-intersubject variability in the Bone_APP from 13.4 of forward rotatation to 23 degrees of backward rotation. Overall, there are no correlation between biometrical parameters and difference of navigated data to others measured on CT. Averaged navigated data was 22.4 degrees. The average anatomic, operative, and planar anteversion were 29.2, 27.2 and 21.3 degrees respectively. The value of anteversion measured on the transverse plane and sagittal plane shows higher than navigated anteversion in paired comparison. This could be comprehended that the navigation system had under-estimated the anteversion than that of transverse and sagittal plane, This means navigation assessed pelvic plane as back ward tilting rather than forward tilting intraoperatively.

None of cases showed the Bone_APP was parallel to conventional coordinate system. Comparing the variable bone APP tilt, all of cases showed an backward tilted soft tissue plane. There were no correlation between bone APP and biometrical parameters. Overall, navigated data were less than anatomic and operative anteversion. Rather than anatomic coordinate system (Bone-APP), backward tilting due to overlying soft tissue (Soft-APP) might to make the navigated data have the tendency to under-estimated the anteversion of cup measured with CT. In conclusion, anterior pelvic plane does not satisfactory reliability with should be easily identified during operation. Image-free navigation system would take into account variations of individuals including both bone tilt and soft tissue plane.
