Aims

Precise implant positioning, tailored to individual spinopelvic biomechanics and phenotype, is paramount for stability in total hip arthroplasty (THA). Despite a few studies on instability prediction, there is a notable gap in research utilizing artificial intelligence (AI). The objective of our pilot study was to evaluate the feasibility of developing an AI algorithm tailored to individual spinopelvic mechanics and patient phenotype for predicting impingement.

Methods

This international, multicentre prospective cohort study across two centres encompassed 157 adults undergoing primary robotic arm-assisted THA. Impingement during specific flexion and extension stances was identified using the virtual range of motion (ROM) tool of the robotic software. The primary AI model, the Light Gradient-Boosting Machine (LGBM), used tabular data to predict impingement presence, direction (flexion or extension), and type. A secondary model integrating tabular data with plain anteroposterior pelvis radiographs was evaluated to assess for any potential enhancement in prediction accuracy.

Aims

Manual impaction, with a mallet and introducer, remains the standard method of installing cementless acetabular cups during total hip arthroplasty (THA). This study aims to quantify the accuracy and precision of manual impaction strikes during the seating of an acetabular component. This understanding aims to help improve impaction surgical techniques and inform the development of future technologies.

Methods

Posterior approach THAs were carried out on three cadavers by an expert orthopaedic surgeon. An instrumented mallet and introducer were used to insert cementless acetabular cups. The motion of the mallet, relative to the introducer, was analyzed for a total of 110 strikes split into low-, medium-, and high-effort strikes. Three parameters were extracted from these data: strike vector, strike offset, and mallet face alignment.

Aims

It is important to analyze objectively the hammering sound in cup press-fit technique in total hip arthroplasty (THA) in order to better understand the change of the sound during impaction. We hypothesized that a specific characteristic would present in a hammering sound with successful fixation. We designed the study to quantitatively investigate the acoustic characteristics during cementless cup impaction in THA.

Methods

In 52 THAs performed between November 2018 and April 2022, the acoustic parameters of the hammering sound of 224 impacts of successful press-fit fixation, and 55 impacts of unsuccessful press-fit fixation, were analyzed. The successful fixation was defined if the following two criteria were met: 1) intraoperatively, the stability of the cup was retained after manual application of the torque test; and 2) at one month postoperatively, the cup showed no translation on radiograph. Each hammering sound was converted to sound pressures in 24 frequency bands by fast Fourier transform analysis. Basic patient characteristics were assessed as potential contributors to the hammering sound.

Aims

The aim of this study was to identify the optimal lip position for total hip arthroplasties (THAs) using a lipped liner. There is a lack of consensus on the optimal position, with substantial variability in surgeon practice.

Methods

A model of a THA was developed using a 20° lipped liner. Kinematic analyses included a physiological range of motion (ROM) analysis and a provocative dislocation manoeuvre analysis. ROM prior to impingement was calculated and, in impingement scenarios, the travel distance prior to dislocation was assessed. The combinations analyzed included nine cup positions (inclination 30-40-50°, anteversion 5-15-25°), three stem positions (anteversion 0-15-30°), and five lip orientations (right hip 7 to 11 o’clock).

Accurate cup placement in total hip arthroplasty (THA) for the patients with developmental dysplasia of the hip (DDH) is one of the challenges due to distinctive bone deformity. Robotic-arm assisted system have been developed to improve the accuracy of implant placement. This study aimed to compare the accuracy of robotic-arm assisted (Robo-THA), CT-based navigated (Navi-THA), and manual (M-THA) cup position and orientation in THA for DDH. A total of 285 patients (335 hips) including 202 M-THAs, 45 Navi-THAs, and 88 Robo-THA were analyzed. The choice of procedure followed the patient's preferences. Horizontal and vertical center of rotation (HCOR and VCOR) were measured for cup position, and radiographic inclination (RI) and anteversion (RA) were measured for cup orientation. The propensity score-matching was performed among three groups to compare the absolute error from the preoperative target position and angle. Navi-THA showed significantly smaller absolute errors than M-THA in RI (3.6° and 5.4°) and RA (3.8° and 6.0°), however, there were no significant differences between them in HCOR (2.5 mm and 3.0 mm) or VCOR (2.2 mm and 2.6 mm). In contrast, Robo-THA showed significantly smaller absolute errors of cup position than both M-THA and Navi-THA (HCOR: 1.7 mm and 2.9 mm, vs. M-THA, 1.6 mm and 2.5 mm vs. Navi-THA, VCOR:1.7 mm and 2.4 mm, vs. M-THA, 1.4 mm and 2.2 mm vs. Navi-THA). Robo-THA also showed significantly smaller absolute errors of cup orientation than both M-THA and Navi-THA (RI: 1.4° and 5.7°, vs. M-THA, 1.5° and 3.6°, vs. Navi-THA, RA: 1.9° and 5.8° vs. M-THA, 2.1° and 3.8° vs. Navi-THA). Robotic-arm assisted system showed more accurate cup position and orientation compared to manual and CT-based navigation in THA for DDH. CT-based navigation increased the accuracy of cup orientation compared to manual procedures, but not cup position

Aims

Spinopelvic characteristics influence the hip’s biomechanical behaviour. However, to date there is little knowledge defining what ‘normal’ spinopelvic characteristics are. This study aims to determine how static spinopelvic characteristics change with age and ethnicity among asymptomatic, healthy individuals.

Methods

This systematic review followed the Preferred Reporting Items for Systematic Review and Meta-Analyses guidelines to identify English studies, including ≥ 18-year-old participants, without evidence of hip or spine pathology or a history of previous surgery or interventional treatment, documenting lumbar lordosis (LL), sacral slope (SS), pelvic tilt (PT), and pelvic incidence (PI). From a total of 2,543 articles retrieved after the initial database search, 61 articles were eventually selected for data extraction.

Aims

In 2021, Vigdorchik et al. published a large multicentre study validating their simple Hip-Spine Classification for determining patient-specific acetabular component positioning in total hip arthroplasty (THA). The purpose of our study was to apply this Hip-Spine Classification to a sample of Australian patients undergoing THA surgery to determine the local acetabular component positioning requirements. Additionally, we propose a modified algorithm for adjusting cup anteversion requirements.

790 patients who underwent THA surgery between January 2021 and June 2022 were assessed for anterior pelvic plane tilt (APPt) and sacral slope (SS) in standing and relaxed seated positions and categorized according to their spinal stiffness and flatback deformity. Spinal stiffness was measured using pelvic mobility (PM); the ΔSS between standing and relaxed seated. Flatback deformity was defined by APPt <-13° in standing. As in Vigdorchik et al., PM of <10° was considered a stiff spine. For our algorithm, PM of <20° indicated the need for increased cup anteversion. Using this approach, patient-specific cup anteversion is increased by 1° for every degree the patient's PM is <20°.

According to the Vigdorchik simple Hip-Spine classification groups, we found: 73% Group 1A, 19% Group 1B, 5% Group 2A, and 3% Group 2B. Therefore, under this classification, 27% of Australian THA patients would have an elevated risk of dislocation due to spinal deformity and/or stiffness. Under our modified definition, 52% patients would require increased cup anteversion to address spinal stiffness.

The Hip-Spine Classification is a simple algorithm that has been shown to indicate to surgeons when adjustments to acetabular cup anteversion are required to account for spinal stiffness or flatback deformity. We investigated this algorithm in an Australian population of patients undergoing THA and propose a modified approach: increasing cup anteversion by 1° for every degree the patient's PM is <20°.

Aims

Iliopsoas impingement occurs in 4% to 30% of patients after undergoing total hip arthroplasty (THA). Despite a relatively high incidence, there are few attempts at modelling impingement between the iliopsoas and acetabular component, and no attempts at modelling this in a representative cohort of subjects. The purpose of this study was to develop a novel computational model for quantifying the impingement between the iliopsoas and acetabular component and validate its utility in a case-controlled investigation.

Methods

This was a retrospective cohort study of patients who underwent THA surgery that included 23 symptomatic patients diagnosed with iliopsoas tendonitis, and 23 patients not diagnosed with iliopsoas tendonitis. All patients received postoperative CT imaging, postoperative standing radiography, and had minimum six months’ follow-up. 3D models of each patient’s prosthetic and bony anatomy were generated, landmarked, and simulated in a novel iliopsoas impingement detection model in supine and standing pelvic positions. Logistic regression models were implemented to determine if the probability of pain could be significantly predicted. Receiver operating characteristic curves were generated to determine the model’s sensitivity, specificity, and area under the curve (AUC).

Aims

Pelvic discontinuity is a rare but increasingly common complication of total hip arthroplasty (THA). This single-centre study evaluated the performance of custom-made triflange acetabular components in acetabular reconstruction with pelvic discontinuity by determining: 1) revision and overall implant survival rates; 2) discontinuity healing rate; and 3) Harris Hip Score (HHS).

Methods

Retrospectively collected data of 38 patients (39 hips) with pelvic discontinuity treated with revision THA using a custom-made triflange acetabular component were analyzed. Minimum follow-up was two years (mean 5.1 years (2 to 11)).

Aims

Navigation devices are designed to improve a surgeon’s accuracy in positioning the acetabular and femoral components in total hip arthroplasty (THA). The purpose of this study was to both evaluate the accuracy of an optical computer-assisted surgery (CAS) navigation system and determine whether preoperative spinopelvic mobility (categorized as hypermobile, normal, or stiff) increased the risk of acetabular component placement error.

Methods

A total of 356 patients undergoing primary THA were prospectively enrolled from November 2016 to March 2018. Clinically relevant error using the CAS system was defined as a difference of > 5° between CAS and 3D radiological reconstruction measurements for acetabular component inclination and anteversion. Univariate and multiple logistic regression analyses were conducted to determine whether hypermobile (Δsacral slope(SS)stand-sit > 30°), or stiff (SSstand-sit < 10°) spinopelvic mobility contributed to increased error rates.

Aims. In computer simulations, the shape of the range of motion (ROM) of a stem with a cylindrical neck design will be a perfect cone. However, many modern stems have rectangular/oval-shaped necks. We hypothesized that the rectangular/oval stem neck will affect the shape of the ROM and the prosthetic impingement. Methods. Total hip arthroplasty (THA) motion while standing and sitting was simulated using a MATLAB model (one stem with a cylindrical neck and one stem with a rectangular neck). The primary predictor was the geometry of the neck (cylindrical vs rectangular) and the main outcome was the shape of ROM based on the prosthetic impingement between the neck and the liner. The secondary outcome was the difference in the ROM provided by each neck geometry and the effect of the pelvic tilt on this ROM. Multiple regression was used to analyze the data. Results. The stem with a rectangular neck has increased internal and external rotation with a quatrefoil cross-section compared to a cone in a cylindrical neck. Modification of the cup orientation and pelvic tilt affected the direction of projection of the cone or quatrefoil shape. The mean increase in internal rotation with a rectangular neck was 3.4° (0° to 7.9°; p < 0.001); for external rotation, it was 2.8° (0.5° to 7.8°; p < 0.001). Conclusion. Our study shows the importance of attention to femoral implant design for the assessment of prosthetic impingement. Any universal mathematical model or computer simulation that ignores each stem’s unique neck geometry will provide inaccurate predictions of prosthetic impingement. This matched cohort study aims to (a) assess differences in spinopelvic characteristics of patients having sustained a dislocation following THA and a control THA group without dislocation; (b) identify spinopelvic characteristics associated with risk of dislocation and; (c) propose an algorithm to define the optimum cup orientation for minimizing dislocation risk. Fifty patients with a history of THA dislocation (29 posterior-, 21 anterior dislocations) were matched for age, gender, body mass index, index diagnosis, and femoral head size with 100 controls. All patients were reviewed and underwent detailed quasi-static radiographic evaluations of the coronal- (offset; center-of-rotation; cup inclination/anteversion) and sagittal- reconstructions (pelvic tilt, pelvic incidence, lumbar lordosis, pelvic-femoral-angle, cup ante-inclination). The spinopelvic balance (PI-LL), combined sagittal index (CSI= Pelvic-femoral-angle + Cup Anteinclination) and Hip-User-Index were determined. sagittal index (CSI= Pelvic-femoral-angle + Cup Anteinclination) and Hip-User-Index were determined. Parameters were compared between the two groups (2-group analysis) and between controls and per direction of dislocation (3-group analysis). There were marginal coronal differences between the groups. Sagittal parameters (lumbar-lordosis, pelvic-tilt, CSI, PI-LL and Hip-User-Index) differed significantly. PI-LL (>10°) and standing pelvic tilt (>18°) were the strongest predictors of dislocation risk (sensistivity:70%/specificity:70%). All hips with a standing CSI<195° dislocated posteriorly and all with CSI>260° dislocated anteriorly. A CSI between 200–245° was associated with significantly reduced risk of dislocation (OR:6; 95%CI:2.5–15.0; p<0.001). In patients with unbalanced and/or rigid lumbar spine, standing CSI of 215–245° was associated with significantly reduced dislocation risk (OR:10; 95%CI:3.2–29.8; p<0.001). PI-LL and standing pelvic-tilt determined from pre-operative, standing, lateral spinopelvic radiograph can be useful screening tools, alerting surgeons of patients at increased dislocation risk. Measurement of the pelvic-femoral angle pre-operatively provides valuable information to determine the optimum, cup orientation associated with reduced dislocation risk by aiming for a standing CSI of 200–245°

Introduction. The functional ante-inclination (AI) of the cup after total hip arthroplasty (THA) is a key component in the combined sagittal index (CSI) to predict joint stability after THA. To accurately predict AI, we deducted a mathematic algorithm between the radiographic anteversion (RA), radiographic inclincation (RI), pelvic tilting (PT), and AI. The current study aims (1) to validate the mathematic algorithm; (2) to convert the AI limits in the CSI index (standing AI ≤ 45°, sitting AI ≥ 41°) into coronal functional safe zone (CFSZ) and explore the influences of the stand-to-sit pelvic motion (PM) and pelvic incidence (PI) on CFSZ; (3) to locate a universal cup orientation that always fulfill the AI criteria of CSI safe zone for all patients or subgroups of PM(PM ≤ 10°, 10° < PM ≤ 30°, and PM > 30°) and PI (PI≤ 41°, 41°< PI ≤ 62°, and PI >62°), respectively. Methods. A 3D printed phantom pelvic model was designed to simulate changing PT values. An acetabular cup was implanted with different RA, RI, and PT settings using robot assisted technique. We enrolled 100 consecutive patients who underwent robot assisted THA from April, 2019 to June, 2019 in our hospital. EOS images before THA and at 6-month follow-up were collected. AI angles were measured on the lateral view radiographs as the reference method. Mean absolute error (MAE), Bland-Altman analysis and linear regression were conducted to assess the accuracy of the AI algorithm for both the phantom and patient radiographic studies. The 100 patients were classified into three subgroups by PM and PI, respectively. Linear regression and ANOVA analysis were conducted to explore the relationship between the size of CFSZ, and PM and PI, respectively. Intersection of the CFSZ was conducted to identify if any universal cup orientation (RA, RI) existed for the CSI index. Results. The mathematic algorithm for calculating AI based on RI, RA, and PT is highly accurate according to the phantom and patient radiographic study. CFSZ size corresponds linearly with PM (R² = 0.638) and PI (R² = 0.129), respectively. There are significant differences in the size of CFSZ, as well as in the intersection of CFSZ and LSZ, between the subgroups of PM and PI, respectively (P<0.017). There is no universal cup orientations could be identified to fulfill the AI limits of the CSI index for all the 100 patients or any of the three subgroups, according to either PM or PI. Conclusions. The cup target orientation should be individualized. The validated algorithm between AI and RA, RI, and PT parameters can serve as the quantitative tool for patient-specific optimization of functional cup AI in different postures

Introduction. Re-revision due to instability and dislocation can occur in up to 1 in 4 cases following revision total hip arthroplasty (THA). Optimal placement of components during revision surgery is thus critical in avoiding re-revision. Computer-assisted navigation has been shown to improve the accuracy and precision of component placement in primary THA; however, its role in revision surgery is less well documented. The purpose of our study was to evaluate the effect of computer-assisted navigation on component placement in revision total hip arthroplasty, as compared with conventional surgery. Methods. To examine the effect of navigation on acetabular component placement in revision THA, we retrospectively reviewed data from a multi-centre cohort of 128 patients having undergone revision THA between March 2017 and January 2019. An imageless computer navigation device (Intellijoint HIP®, Intellijoint Surgical, Kitchener, ON, Canada) was utilized in 69 surgeries and conventional methods were used in 59 surgeries. Acetabular component placement (anteversion, inclination) and the proportion of acetabular components placed in a functional safe zone (40° inclination/20° anteversion) were compared between navigation assisted and conventional THA groups. Results. Mean inclination decreased post-operatively versus baseline in both the navigation (44.9°±12.1° vs. 43.0°±6.8°, p=0.65) and control (45.8°±19.4° vs. 42.8°±7.1°, p=0.08) groups. Mean anteversion increased in both study groups, with a significant increase noted in the navigation group (18.6°±8.5° vs. 21.6°±7.8°, p=0.04) but not in the control group (19.4°±9.6° vs. 21.2°±9.8°, p=0.33). Post-operatively, a greater proportion of acetabular components were within ±10° of a functional target (40° inclination, 20° anteversion) in the navigation group (inclination: 59/67 (88%), anteversion: 56/67 (84%)) than in the control group (49/59 (83%) and 41/59, (69%), respectively). Acetabular component precision in both study groups improved post-operatively versus baseline. Variance in inclination improved significantly in both control (50.6° vs. 112.4°, p=0.002) and navigation (46.2° vs. 141.1°, p<0.001) groups. Anteversion variance worsened in the control group (96.3° vs. 87.6°, p=0.36) but the navigation group showed improvement (61.2° vs. 72.7°, p=0.25). Post-operative variance amongst cup orientations in the navigation group (IN: 46.2°; AV: 61.2°) indicated significantly better precision than that observed in the control group (IN: 50.6°, p=0.36; AV: 96.3°, p=0.04). Discussion. Re-revision is required in up to 25% of revision THA cases, of which 36% are caused by instability. This places a significant burden on the health care system and highlights the importance of accurate component placement. Our data indicate that the use of imageless navigation in revision THA – by minimizing the likelihood of outliers – may contribute to lower rates of re-revision by improving component orientation in revision THA. Conclusion. Utilizing imageless navigation in revision THAs results in more consistent placement of the acetabular component as compared to non-navigated revision surgeries

Abstract. Objectives. Accurate orientation of the acetabular component during a total hip replacement is critical for optimising patient function, increasing the longevity of components, and reducing the risk of complications. This study aimed to determine the validity of a novel VR platform (AescularVR) in assessing acetabular component orientation in a simulated model used in surgical training. Methods. The AescularVR platform was developed using the HTC Vive® VR system hardware, including wireless trackers attached to the surgical instruments and pelvic sawbone. Following calibration, data on the relative position of both trackers are used to determine the acetabular cup orientation (version and inclination). The acetabular cup was manually implanted across a range of orientations representative of those expected intra-operatively. Simultaneous readings from the Vicon® optical motion capture system were used as the ‘gold standard’ for comparison. Correlation and agreement between these two methods was determined using Bland-Altman plots, Pearson's correlation co-efficient, and linear regression modelling. Results. A total of 55 separate orientation readings were obtained. The mean average difference in acetabular cup version and inclination between the Vicon and VR systems was 3.4° (95% CI: −3–9.9°), and −0.005° (95% CI: −4.5–4.5°) respectively. Strong positive correlations were demonstrated between the Vicon and VR systems in both acetabular cup version (Pearson's R = 0.92, 99% CI: 0.84–0.96, p<0.001), and inclination (Pearson's R = 0.94, 99% CI: 0.88–0.97, p<0.001). Using linear regression modelling, the adjusted R. 2. for acetabular version was 0.84, and 0.88 for acetabular inclination. Conclusion. The results of this study indicate that the AescularVR platform is highly accurate and reliable in determining acetabular component orientation in a simulated environment. The AescularVR platform is an adaptable tracking system, which may be modified for use in a range of simulated surgical training and educational purposes, particularly in orthopaedic surgery. Abstract. Objectives. The importance of cup position on the performance of total hip replacements (THR) has been demonstrated in in vitro hip simulator tests and clinically. However, how cup position changes during gait has not been considered and may affect failure scenarios. The aim of this study was to assess dynamic cup version using gait data. Methods. Pelvic movement data for walking for 39 unilateral THR patients was acquired (Leeds Biomedical Research Centre). Patient's elected walking speed was used to group patients into high- and low-functioning (mean speed, 1.36(SD 0.09)ms. −1. and 0.85(SD 0.08)ms. −1. respectively). A computational algorithm (Python3.7) was developed to calculate cup version during gait cycle. Inputs were pelvic angles and initial cup orientation (assumed to be 45° inclination and 7° version, anterior pelvic plane was parallel to radiological frontal plane). Outputs were cup version angles during a gait cycle (101 measurements/cycle). Minimum, maximum and average cup version during gait cycle were measured for each patient. Two-sample t-test (p=0.05) was used to compare groups. Results. Over a gait cycle the mean minimum, maximum and average version angles for the high-functioning group were −4.5(SD 4.4)°, 5.0(SD 4.3)°, 9.5(SD 4.0)° and for low-functioning group 2.0(SD 3.7)°, 6.2(SD 2.9)°, 8.1(SD 3.2)°. There were no significant differences for the minimum, maximum and average version angles between the two groups. Conclusions. The study shows that dynamic acetabular cup version changes substantially during gait and this must be considered clinically and in pre-clinical testing. There was no significant difference between the two groups; however, dynamic cup version was more negative in high-functioning compared to low-functioning patients. Further studies on a larger cohort are required to determine whether patients’ profiles can be stratified to provide enhanced inputs for pre-clinical THR testing. Declaration of Interest. Introduction. Excessive standing posterior pelvic tilt (PT), lumbar spine stiffness, low pelvic Incidence (PI), and severe sagittal spinal deformity (SSD) have been linked to increased dislocation rates. We aimed to compare the prevalence of these 4 parameters in unstable and stable primary Total Hip Arthroplasty (THA) patients. Methods. In this retrospective cohort study, 40 patients with instability following primary THA for osteoarthritis were referred for functional analysis. All patients received lateral X-rays in standing and flexed seated positions to assess functional pelvic tilt and lumbar lordosis (LL). Computed tomography scans were used to measure pelvic incidence and acetabular cup orientation. Literature thresholds for “at risk” spinopelvic parameters were standing pelvic tilt ≤ −10°, lumbar flexion (LL. stand. – LL. seated. ) ≤ 20°, PI ≤ 41°, and sagittal spinal deformity (PI – LL. stand. mismatch) ≥ 10°. The prevalence of each risk factor in the dislocation cohort was calculated and compared to a previously published cohort of 4042 stable THA patients. Results. Median supine cup inclination for the dislocating cohort was 43° (range, 26°- 58°). Median cup anteversion was 23° (range, 7° − 40°) for the dislocating cohort. 65% of the dislocating patients had socket positions within the Lewinnek safe zone (Figure 1). Standing PT (-10° v −1°), lumbar flexion (20° v 45°), and PI-LL mismatch (12° v −1°) were all significantly different (p < 0.001) in the dislocating group compared to the stable THA population (Figure 2). There was no difference in PI between the dislocating group and the stable THA population (58° v 56° respectively, p = 0.33), with the numbers available. 80% of the dislocating patients had one or more of the 3 statistically significant risk factors, compared to only 24% of the stable THA population. Conclusion. Excessive standing posterior pelvic tilt, low lumbar flexion and a severe SSD are highly prevalent in unstable THAs. Introduction. Lumbar spine fusion in patients undergoing THA (total hip arthroplasty) is a known risk factor for hip dislocation with some studies showing a 400% increased incidence compared to the overall THA population. Reduced spine flexibility can effectively narrow the cup anteversion safe zone while alterations in pelvic tilt can alter the center of the anteversion safe zone. The use of precision cup alignment technology combined with patient-specific cup alignment goals based on preoperative assessment has been suggested as a method of addressing this problem. The current study assess the dislocation rate of THA patients with stiff or fused lumbar spines treated using surgical navigation with patient-specific cup orientation goals. Methods. Seventy-five THA were performed in 54 patients with a diagnosis of lumbar fusion, lumbar disc replacement, and scoliosis with Cobb angles greater than 40 degrees were treated by the senior author (SM) as part of a prospective, non-randomized study of surgical navigation in total hip arthroplasty. All patients were treated using a smart mechanical navigation tool for cup alignment (HipXpert System, Surgical Planning Associates, Inc., Boston, MA). Cup orientation goals were set on a patient-specific basis using supine pelvic tilt as measured using CT. Patients with increased pelvic tilt had a goal for increased cup anteversion and patients with decreased pelvic tilt had a goal for decreased cup anteversion (relative to the anterior pelvic plane coordinate system). Each patient's more recent outpatient records were assessed for history of dislocation, instability, mechanical symptoms, decreased range of motion or progressive pain. Additionally, last clinic radiographs were reviewed to confirm lumbar pathology in the form of spinal surgical hardware. Results. Seventy-five total hip arthroplasties with stiff lumbar spine were reviewed with and average follow up of 6.04 years. The average number of levels of lumbar fusion was 2.3 levels. Since the most recent follow up on all patients in this cohort no hip dislocations had occurred. Discussion and Conclusion. Fusion or stiffness of the lumbar spine is a known risk factor for instability following elective THA. The current study demonstrates that patient-specific planning of cup placement taking abnormal pelvic tilt into consideration combined with the use of accurate intra-operative cup alignment technology can be used to address this problem

Introduction. Interactions between hip, pelvis and spine, as abnormal spinopelvic movements, have been associated with inferior outcomes following total hip arthroplasty (THA). Changes in pelvis position lead to a mutual change in functional cup orientation, with both pelvic tilt and rotation having a significant effect on version. Hip osteoarthritis (OA) patients have shown reduced hip kinematics which may place increased demands on the pelvis and the spine. Sagittal and coronal planes assessments are commonly done as these can be adequately studied with anteroposterior and lateral radiographs. However, abnormal pelvis rotation is likely to compromise the outcome as they have a detrimental effect on cup orientation and increased impingement risk. This study aims to determine the association between dynamic motion and radiographic sagittal assessments; and examine the association between axial and sagittal spinal and pelvic kinematics between hip OA patients and healthy controls (CTRL). Methods. This is a prospective study, IRB approved. Twenty hip OA pre-THA patients (11F/9M, 67±9 years) and six CTRL (3F/3M, 46±18 years) underwent lateral spinopelvic radiographs in standing and seated bend-and-reach (SBR) positions. Pelvic tilt (PT), pelvic-femoral-angle (PFA) and lumbar lordosis (LL) angles were measured in both positions and the differences (Δ) between standing and SBR were calculated. Dynamic SBR and seated maximal-trunk-rotation (STR) were recorded in the biomechanics laboratory using a 10-infrared camera and processed on a motion capture system (Vicon, UK). Direct kinematics extracted maximal pelvic tilt (PT. max. ), hip flexion (HF. max. ) and (mid-thoracic to lumbar) spinal flexion (SF. max. ). The SBR pelvic movement contribution (ΔPT. rel. ) was calculated as ΔPT/(ΔPT+ΔPFA)∗100 for the radiographic analysis and as PT. max. /(PT. max. +HF. max. ) for the motion analyses. Axial and sagittal, pelvic and spinal range of motion (ROM) were calculated for STR and SBR, respectively. Spearman's rank-order determined correlations between the spinopelvic radiographs and sagittal kinematics, and the sagittal/axial kinematics. Mann-Whitney U-tests compared measures between groups. Results. Radiograph readings correlated with sagittal kinematics during SBR for ΔPT and PT. max. (ρ=0.64, p<0.001), ΔPFA and HF. max. (ρ=0.44, p<0.0002), and ΔLL and SF. max. (ρ=0.34, p=0.002). Relative pelvic movements (ΔPT. rel. ) were not different between radiographic (11%±21) and biomechanical (15%±29) readings (p=0.9). Sagittal SRB spinal flexion correlated with the axial STR rotation (ρ=0.43, p<0.0001). Although not seen in CTRL, sagittal SRB pelvic flexion strongly correlated with STR pelvic rotation in OA patients (ρ=0.40, p=0.002). All spinopelvic parameters were different between the patients with OA and CTRL. CTRLs exhibited significantly greater mobility and less variability in all 3 segments (spine, pelvis, hip) and both planes (axial and sagittal) (Table 1). Conclusion. Correlation between sagittal kinematics and radiographical measurements during SBR validates the spinopelvic mobility assessments in the biomechanics laboratory. Axial kinematics of both pelvis and spine correlated significantly in OA patients, suggesting that patients with abnormal sagittal mobility are likely to also exhibit abnormal axial mobility, which can further potentiate any at-risk kinematics. Significantly lower OA ROM must be investigated post-THA. Pre-THA variability of both sagittal and axial movements indicates that both planes must be considered ahead of surgical planning with navigation and/or robotics.
