Total temporomandibular joint (TMJ) replacements reduce pain and improve quality of life in patients suffering from end-stage TMJ disorders, such as osteoarthritis and trauma. Jaw kinematics measurements following TMJ arthroplasty provide a basis for evaluating implant performance and jaw function. The aim of this study is to provide the first measurements of three-dimensional kinematics of the jaw in patients following unilateral and bilateral prosthetic TMJ surgeries. Jaw motion tracking experiments were performed on 7 healthy control participants, 3 unilateral and 1 bilateral TMJ replacement patients. Custom-made mouthpieces were manufactured for each participant's mandibular and maxillary teeth, with each supporting three retroreflective markers anterior to the participant's lip line. Participants performed 15 trials each of maximum jaw opening, lateral and protrusive movements. Marker trajectories were simultaneously measured using an optoelectronic tracking system. Laser scans taken of each dental plate, together with CT scans of each patient, were used to register the plate position to each participant's jaw geometry, allowing 3D condylar motion to be quantified from the marker trajectories. The maximum mouth opening capacity of joint replacement patients was comparable to healthy controls with average incisal inferior translations of 37.5mm, 38.4mm and 33.6mm for the controls, unilateral and bilateral joint replacement patients respectively. During mouth opening the maximum anterior translation of prosthetic condyles was 2.4mm, compared to 10.6mm for controls. Prosthetic condyles had limited anterior motion compared to natural condyles, in unilateral patients this resulted in asymmetric opening and protrusive movements and the capacity to laterally move their jaw towards their pathological side only. For the bilateral patient, protrusive and lateral jaw movement capacity was minimal. Total TMJ replacement surgery facilitates normal mouth opening capacity and lateral and inferior condylar movements but limits anterior condylar motion. This study provides future direction for TMJ implant design

Aims: Cobalt (Co) and chromium (Cr) ion associated carcinogenesis and chromosomal damage in animals have raised concerns that metal-on-metal (MOM) total hip replacement (THR) in humans may produce the same effects over time. Considering that the risks may be related to the level of these ions in the body, this study compared the serum Co and Cr levels in patients with unilateral versus bilateral 28 mm diameter MOM THR. Methods: All patients having THR at our institution were prospectively registered on a computerised database. From our database, 108 Ultima MOM THR with 28 mm CoCrMo bearing were identified. After patient review in clinic and before blood results were known, patient matching was performed by date after surgery, activity level and weight. Using these stringent criteria, 11 unilateral THR could be adequately matched with 11 bilateral THR. Blood serum was taken with full anti-contamination protocols and serum analysed via inductively coupled plasma mass spectrometry (ICP-MS) Statistical analysis used the Mann-Whitney U test. Results: The median serum Co level after unilateral MOM THR was 22 nmol (range 15 to 37 nmol) compared to 42 nmol (range 19 to 221 nmol) for bilateral MOM THR (p=0.001). The median serum Cr level after unilateral MOM THR was 19 nmol (range 2 to 35 nmol) compared to 52 nmol (range 19 to 287 nmol) for bilateral MOM THR (p=0.04). Conclusions: This study has shown that the serum Co and Cr levels in patients with bilateral MOM THR are significantly higher than those with unilateral MOM THR. With levels of up to 50 times the upper of limit of normal, this finding may be of relevance for the development of potential long-term side effects

Aims: Elevated serum cobalt and chromium ion levels associated with carcinogenesis and chromosomal damage in animals has raised concerns that metal-on-metal (MOM) total hip replacement (THR) in humans may produce the same effects over time. Considering that the risks may be related to the level of these ions in the body, this study compared the serum cobalt and chromium ion levels in patients with unilateral versus bilateral 28 mm diameter MOM THR. From our database, we identified 108 patients with Ultima (Johnson and Johnson, Leeds) MOM THR with 28 mm bearing made of cobalt-chromium alloy. Using these stringent criteria, 11 unilateral THR could be adequately matched with 11 bilateral THR. Blood serum was taken with full anti-contamination protocols and serum analysed via inductively coupled plasma mass spectrometry. Statistical analysis used the Mann-Whitney U test. Results: The serum cobalt ion level after unilateral MOM THR was 4.4 times normal (median 22 nmol/L, range 15 to 37 nmol/L) compared to 8.4 times normal (median 42 nmol/L, range 19 to 221 nmol/L) for bilateral MOM THR (p=0.001). The serum chromium ion level after unilateral MOM THR was 3.8 times normal (median 19 nmol/L, range 2 to 35 nmol/L) compared to 10.4 times normal (median 52 nmol/L, range 19 to 287 nmol/L) for bilateral MOM THR (p=0.04). Conclusions: This study has shown that the serum cobalt and chromium ion levels in patients with bilateral MOM THR are significantly higher than those in patients with unilateral MOM THR. With levels of up to 50 times the upper limit of normal, this finding may be of relevance for the potential development of long-term side effects

Purpose of the study. To compare the effectiveness of unilateral and bilateral pedicle screw techniques in correcting adolescent idiopathic scoliosis. Summary of Background Data. Pedicle screw constructs have been extensively used in the treatment of adolescent patients with idiopathic scoliosis. It has been suggested that greater implant density may achieve better deformity correction. However, this can increase the neurological risk related to pedicle screw placement, prolong surgical time and blood loss and result in higher instrumentation cost. Methods. We reviewed the medical notes and radiographs of 139 consecutive adolescent patients with idiopathic scoliosis (128 female-11 male, prospectively collected single surgeon's series). We measured the scoliosis, thoracic kyphosis (T5-T12), and lumbar lordosis (L1-L5) before and after surgery, as well as at minimum 2-year follow-up. SRS 22 data was available for all patients. Results. All patients underwent posterior spinal arthrodesis using pedicle screw constructs. Mean age at surgery was 14.5 years. We had 2 separate groups: in Group 1 (43 patients) correction was performed over 2 rods using bilateral segmental pedicle screws; in Group 2 (96 patients) correction was performed over 1 rod using unilateral segmental pedicle screws with the 2. nd. rod providing stability of the construct through 2-level screw fixation both proximal and distal. Group 1. Mean Cobb angle before surgery for upper thoracic curves was 37°. This was corrected by 71% to mean 11° (p<0.001). Mean Cobb angle before surgery for main thoracic curves was 65°. This was corrected by 71% to mean 20° (p<0.001). Mean Cobb angle before surgery for thoracolumbar/lumbar curves was 60°. This was corrected by 74% to mean 16° (p<0.001). No patient lost >2° correction at follow-up. Mean preoperative thoracic kyphosis was 24° and lumbar lordosis 52°. Mean postoperative thoracic kyphosis was 21° and lumbar lordosis 50° (p>0.05). Mean theatre time was 5.5 hours, hospital stay 8.2 days and intraoperative blood loss 0.6 blood volumes. Complications: 1 transient IOM loss/no neurological deficit; 1 deep wound infection leading to non-union and requiring revision surgery; 1 rod trimming due to prominent upper end. Mean preoperative SRS 22 score was 3.9; this improved to 4.5 at follow-up (p<0.001). Pain and self-image demonstrated significant improvement (p=0.001, p<0.001 respectively) with mean satisfaction rate 4.9. Group 2. Mean Cobb angle before surgery for upper thoracic curves was 42°. This was corrected by 52% to mean 20° (p<0.001). Mean Cobb angle before surgery for main thoracic curves was 62°. This was corrected by 70% to mean 19° (p<0.001). Mean Cobb angle before surgery for thoracolumbar/lumbar curves was 57°. This was corrected by 72% to mean 16° (p<0.001). No patient lost >2° correction at follow-up. Preoperative scoliosis size for all types of curves correlated with increased surgical time (r=0.6, 0.4). Mean preoperative thoracic kyphosis was 28° and lumbar lordosis 46°. Mean postoperative thoracic kyphosis was 25° and lumbar lordosis 45° (p>0.05). Mean theatre time was 4.2 hours, hospital stay 8.4 days and intraoperative blood loss 0.4 blood volumes. Complications: 1 deep and 1 superficial wound infections treated with debridement; 1 transient brachial plexus neurapraxia; 1 SMA syndrome. Mean preoperative SRS 22 score was 3.7; this improved to 4.5 at follow-up (p<0.001). Pain, function, self-image and mental health demonstrated significant improvement (p<0.001 for all parameters) with mean satisfaction rate 4.8. Comparison between groups showed no significant difference in regard to age at surgery, preoperative and postoperative scoliosis angle for main thoracic and thoracolumbar/lumbar curves, as well as SRS scores and length of hospital stay. Better correction of upper thoracic curves was achieved in Group 1 (p<0.05), but upper thoracic curves in Group 2 were statistically more severe before surgery (p<0.05). Increased surgical time and blood loss was recorded in Group 1 (p<0.05, p=0.05 respectively). The implant cost was reduced by mean 35% in Group 2 due to lesser number of pedicle screws. Conclusion. Unilateral and bilateral pedicle screw instrumentation has achieved excellent deformity correction in adolescent patients with idiopathic scoliosis, which was maintained at follow-up. This has been associated with high patient satisfaction and low complication rates. The unilateral technique using segmental pedicle screw correction has reduced surgical time, intraoperative blood loss and implant cost without compromising surgical outcome for the most common thoracic and thoracolumbar/lumbar curves. The bilateral technique achieved better correction of upper thoracic scoliosis

Europeam Group of Neuro-orthopaedic (GLAENeO), Caracas, The prevention of a dislocated hip is one of the aims of early surgery in Cerebral Palsy children, specially those severely involved. We performed a retrospective study of those cerebral palsy patients operated of adductor tenotomy between 1975 and 1995 with a total of 1474 patients. We grouped them in those who had a unilateral tenotomy and those who had a bilateral tenotomy as primary surgery. Of these only 8% had an obturator neurectomy, without walking ability, and 92 % had it not. Age at surgery varied from 6 months to 8 years of age with a mean of 4 years and 3 months. Group I: 792 patients (53.7 %) with unilateral adductor contracture, sustained a unilateral adductor tenotomy. Of these patients a total of 619 (78, 2 %) required a contralateral adductor tenotomy at a mean of 3 years and 6 months. Group II: 682 patients (46, 3 %) with bilateral adductor contracture that had a bilateral adductor tenotomy in one stage. Of the 792 patients that sustained a two stage adductor tenotomy, 123 (20%) presented a unilateral dislocated hip and of these 115 (93 %) occurred in the hip operated secondly at a mean of 1 year post tenotomy. Of the 682 patients with bilateral adductor tenotomies only 7 (1 %) had a dislocated hip 2 years post tenotomy. Of the 72 dislocated hips, 12 (59 %) were quadriplegics, 28 (22 %) were diplegic, 21 (18 %) hemiplegics and 1 (1 %) tetraplegic.

Of the 619 patients tenotomized in two stages, in 143 the diaphyseal – cervical angle was 155 ° (23,1 %), at a mean of 6 and a half years of age and 3 years post the second tenotomy. In 102 of these patients (71 %) a varus derotation osteotomy was performed in the hip operated in the second act with further dislocation of the hip in 20 cases (20 %). Of the 685 patients with bilateral tenotomy in one stage, varus derotation osteotomy was required in 68 (68 %) at a mean of 6 years of age with only a 3 % of dislocations in this group.

In view of these results we recommend a bilateral adductor tenotomy be performd regardless of a difference in the degree of contracture of both sides, thus coordinating the forces and avoid further dislocation the hip.

Background. To determine the relative contributions of bilateral versus unilateral femoral shaft fracture plus injuries in other body regions to mortality after injury. Study design. A retrospective analysis of the prospectively recorded Trauma Registry data (TARN) from 1989 to 2003. Methods. Patients were divided into groups UFi (isolated unilateral femur injury),BFi (isolated bilateral femur injury) and UFa and BFa if an associated injury was present. Data collected for each patient included age, Injury Severity Scores, Glasgow Coma Scale, mortality, physiological parameters, the timing and extent of prehospital care, the time to arrival at the hospital, initial treatments, time to and type of surgery, length of ICU and hospital stay. Logistic regression data analysis was performed to determine variables that were associated with increased mortality. Results. Patients in group BFa had a significantly higher ISS (23 vs 9),reduced GCS (12 vs 15) and increased mortality rate (31.6% vs 9.8%) than patients in group BFi. Group BFa patients had an increased number of associated injuries than group UFa. Regression analysis of variables evident on admission revealed a significant correlation between bilateral femoral fractures with associated injuries and mortality. However bilateral fracture, even in isolation significantly increased the odds of mortality by 3.07(1.36-6.92). Intramedullary nailing (IMN) was the method of fracture fixation associated with the lowest patient mortality overall. However, when assessing patient mortality in the BFa group with an ISS of more than 40, three other fracture fixation regimens were associated with a lower mortality rate than the IMN group. Conclusions. The increase in mortality with bilateral femoral fractures is more closely associated with the presence of associated injuries and poor physiological parameters than to the presence of the bilateral femoral fracture alone. However contrary to ISS scoring the latter does convey a significant additional risk of mortality. The presence of bilateral femoral fractures should alert the clinician to the very high likelihood (80%) of significant associated injuries in other body systems and their life-threatening potential

Purpose of the study: Acetabular version is a most important parameter for repositioning the acetabular fragment during periacetabular osteotomy. Recently, a few studies have presented a significant number of dysplastic hips with acetabular retroversion. There have not however been any studies devoted specifically to the severity of bilateral acetabular retroversion. The purpose of this work was to determine the incidence of bilateral retroversion in patients undergoing periacetabular osteotomy for dysplasia in order to identify and validate a retroversion index which would be predictive of the degree of retroversion. This index could be added to congenital hip dysplasia classifications to include acetabular version.

Material and methods: The Lequesne lateral view of the hip was obtained in 174 patients (348 hips, 137 women and 37 men, mean age 30 years) undergoing periacetabular osteotomy for symptomatic dysplasia. One hundred ninety-five hips (56%) were operated on and 153 (44%) were considered normal or non-symptomatic and were not operated. The following parameters were noted for each hip: VCE, VCA, HTE, femoral head extrusion, index of acetabular depth, crossing-over, retroversion index. The retroversion index was checked on a bone model of the pelvis which was x-rayed in the neutral position then turned progressively. Statistical data were analyzed with SAS.

Results: Five percent of the operated hips presented neutral version, 53% anteversion and 42% retroversion. Twenty-four percent of the non-operated hips were normal, 22% presented pure retroversion and 54% were dysplastic. All of the measurements were significantly deviated towards dysplasia for operated hips, with the exception of the retroversion index and the VCA.

Discussion: These data validated the retroversion index and confirmed that one out of three dysplastic hips displays retroversion. In addition, it would appear that for dysplastic hips with retroversion, the degree of lateral coverage or the HTE angle determines whether surgery is needed or not and not the degree of retroversion. But as pure retroversion can be symptomatic in itself, and since the majority of these version or cover anomalies can be treated by periacetabular osteotomy, we propose a classification of hip dysplasia included acetabular version.

Conclusion: This classification is designed as an aid for the orthopedic surgeon for reorienting the acetabular fragment to obtain the optimal position.

Many studies have evaluated bilateral versus unilateral surgery in large joints, however, limited research is available to compare outcomes of bilateral-staged foot surgeries versus synchronous-bilateral foot surgery. 186 consecutive cases of first metatarsophalangeal joint surgery were prospectively included in this study; 252 procedures were performed: 120 were unilateral or staged-bilateral, and 66 were synchronous-bilateral operations. Patients were evaluated at 6–and 12-weeks for specific early complications, and surveyed about there return to work, activities of daily living, shoe gear requirements, satisfaction, and reasons for choosing staged or synchronous surgery. Additionally, a cost analysis was performed on all surgical scenarios. Student-t test showed no statistical significance between groups in all clinical settings to a 95% confidence level. Complication rates were similar and few in all situations. Patients were very satisfied when choosing bilateral-synchronous surgery and would elect to repeat it the same way 97% of the time. The economic costs to the health system average 25% greater when patients undergoing first metatarsophalangeal joint surgery have the procedure performed one foot at a time. Combined with the time lost from work, this reveals a significant economic cost to both society and patient

The emergence of patient specific instrumentation has seen an expansion from simple radiographs to plan total knee arthroplasty (TKA) with modern systems using computed tomography (CT) or magnetic resonance imaging scans. Concerns have emerged regarding accuracy of these non-weight bearing modalities to assess true mechanical axis. The aim of our study was to compare coronal alignment on full length standing AP imaging generated by the EOS acquisition system with the CT coronal scout image. Eligible patients underwent unilateral or bilateral primary TKA for osteoarthritis under the care of investigating surgeon between 2017 and 2022, with both EOS X-Ray Imaging Acquisition System and CT scans performed preoperatively. Coronal mechanical alignment was measured on the supine coronal scout CT scan and the standing HKA EOS. Pre-operative lower limb coronal alignment was assessed on 96 knees prior to TKA on the supine coronal scout CT scan and the standing HKA EOS. There were 56 males (56%), and 44 right knees (44%). The mean age was 68 years (range 53-90). The mean coronal alignment was 4.7 degrees (SD 5.3) on CT scan and 4.6 degrees (SD 6.2) on EOS (p=0.70). There was a strong positive correlation of coronal alignment on CT scan and EOS (pearson. 0.927, p=0.001). The mean difference between EOS and CT scan was 0.9 degrees (SD 2.4). Less than 3 degrees variation between measures was observed in 87% of knees. On linear regression for every 1° varus increase in CT HKA alignment, the EOS HKA alignment increased by 0.93° in varus orientation. The model explained 86% of the variability. CT demonstrates excellent reliability for assessing coronal lower limb alignment compared to EOS in osteoarthritic knees. This supports the routine use of CT to plan TKA without further weight bearing imaging in routine cases

Since its creation, labral repair has become the preferred method among surgeons for the arthroscopic treatment of acetabular labral tears resulting in pain and dysfunction for patients. Labral reconstruction is performed mainly in revision hip arthroscopy but can be used in the primary setting when the labrum cannot be repaired or is calcified. The purpose of this study was to compare the survival between primary labral repair and labral reconstruction with survival defined as no further surgery (revision or total hip replacement). Patients who underwent labral repair or reconstruction between January 2005 and December 2018 in the primary setting were included in the study. Patients were included if they had primary hip arthroscopy with the senior author for femoroacetabular impingement (FAI), involving either labral reconstruction or labral repair, and were within the ages of 18 and 65 at the time of surgery. Exclusion criteria included confounding injuries (Leggs Calves Perthes, avascular necrosis, femoral head fracture, etc.), history of unilateral or bilateral hip surgeries, or Tönnis grades of 2 or 3 at the time of surgery. Labral repairs were performed when adequate tissue was available for repair and labral reconstruction was performed when tissue was absent, ossified or torn beyond repair. A total of 501 labral repairs and 114 labral reconstructions performed in the primary setting were included in the study. Labral reconstruction patients were older (37±10) compared to labral repair (34±11).(p=0.021). Second surgeries were required in 19/114 (17%) of labral reconstruction and 40/501(8%) [odds ratio: 2.3; 95% CI 1.3 to 4.2] (p=0.008). Revision hip arthroscopy were required in 6/114(5%) labral reconstructions and 33/501(6.5%) labral repair (p=0.496). Total hip replacement was required in 13/114 labral reconstructions and 7/501 labral repairs [odds ratio:9.1 95%CI 3.5 to 23] (p=< 0.01). The mean survival for the labral repair group was 10.2 years (95%CI:10 to 10.5) and 11.9 years (98%CI:10.9 to 12.8) in the labral reconstruction group. Conversion to total hip was required more often following primary labral reconstruction. Revision hip arthroscopy rates were similar between groups as was the mean survival, with both over 10 years. Similar survival was seen in labral repair and reconstruction when strict patient selection criteria are followed

Introduction. Acetabular dysplasia is typically characterised by insufficient antero-superior femoral head coverage. It is postulated (yet unproven) that patients with dysplasia compensate by reducing pelvic tilt (anterior pelvic rotation), effectively retroverting their acetabulum to improve antero-superior cover. We aimed to 1) define pelvic tilt (PT) in patients with bilateral and unilateral dysplasia, and 2) quantify PT changes following a successful periacetabular osteotomy (PAO). Patients/Materials and Methods. We retrospectively reviewed 16 patients (14 females) who underwent successful, bilateral, staged, PAOs (32 PAOs). These cases were matched for age and gender with 32 unilaterally dysplastic hips that underwent successful PAO for a similar degree of acetabular dysplasia as per pre-operative imaging. Supine and AP radiographs pre-PAO and at latest follow-up were used to measure centre-edge-angle (CEA) and Tönnis-angle (TA). PT was measured using two validated methods (Sacro-Femoral-Pubic (SFP) angle and Pubic-Symphysis to Sacro-Iliac (PS-SI) Index with excellent correlation (ρ=0.8, p<0.001). Results. Post-operatively, the TA/CEA improved from 20°±8/11°±9 to 5°±5/33°±6, respectively, with similar improvements between the unilateral and bilateral hips (p=0.9). PT pre-operatively was similar between unilaterally (8°±5) and bilaterally dysplastic hips (7°±5) (p=0.87). The change in PT was −1°±3; changing by >5° (all increased) in 6 patients (13%; 3 with unilateral PAOs and 3 with bilateral PAOs). Discussion. Patients with unilateral and bilateral hip dysplasia demonstrate similarities in PT. In 87% of cases PT remained the same following PAO, therefore was unlikely to have been compensatory. PT increased in 13% of cases post-PAO illustrating that a change in PT is possible and suggesting that the reduced tilt pre-PAO may have been compensatory to improve functional femoral head coverage in these patients. Conclusion. This is the first clinical and radiological evidence that patients with bilateral or unilateral dysplasia demonstrate similar pre- and post-operative PT, which remains unchanged in almost 90% of cases

Purpose. The aim of this study was to identify if perioperative outcomes were different in patients with cerebral palsy undergoing unilateral or bilateral hip reconstruction. Method. All consecutive hip reconstructions for cerebral palsy performed by the senior author (FNT) within a tertiary-referral centre were identified between January 2012 and July 2016. Patients were stratified by age, gender, GMFCS and side of procedure. Length of surgery, pre- and post- operative haemoglobin, length of stay and immediate post-operative complications were measured. Results. 158 procedures were performed; 96 bilateral and 62 unilateral (37 left, 25 right). In bilateral procedures, operative time was 14 minutes longer (p= 0.07) and haemoglobin decrease was 1.85g/l greater (p= 0.34) with equivalent length of stay (average 6.4 days) and complication rate when compared to those undergoing unilateral procedures. Irrespective of procedure, those with a GMFCS level V had a longer length of stay (p< 0.05) and post-operative complication rate (p< 0.05) when compared to patients with GMFCS level IV. Conclusion. No significant differences were noted in the outcome measures when comparing unilateral and bilateral hip reconstructions in our group of patients with cerebral palsy. This suggests that concurrent prophylactic surgical management of the radiologically stable hip is a safe surgical option in this high-risk group of paediatric patients in an appropriate tertiary care setting with multidisciplinary team support

Background. The current average tariff of a total knee replacement (TKR) is £5500. The approximate cost of each knee prosthesis is £2500. Therefore, length of patient stay (LOS) and the cost of patient rehabilitation influence the total costs significantly. Previous studies have shown a mean LOS of between 5 and 9.4 days for patients undergoing primary unilateral TKR but none looked at the factors influencing length of stay following bilateral primary total knee replacements (BTKR) at the same sitting. Objectives. To identify significant factors that influence the LOS following BTKR at the same sitting in a single centre in the UK. Methods. This was a retrospective single-centre study performed at the Princess Royal Hospital which performed a total of 25 BTKR. Surgical and patient factors that may influence LOS were recorded and analysed. Results. The mean LOS was 10 days with a median of 9 days. 64% were discharged within 10 days. Those staying longer were classified as long stayers. Being a female (0.65, p< 0.05), having a higher Charlson index (0.68, p< 0.05) and having a post-operative blood transfusion (0.59, p< 0.05) were the only significant factors that influenced LOS. Post-operative acute kidney injury (AKI), underlying diagnosis such as rheumatoid arthritis, BMI, age, worse pre-operative oxford knee scores and type of implant did not influence LOS. Conclusion. Factors influencing LOS following BTKR shown in our study seems to be the same as those influencing unilateral TKRs as identified in the literature. This should be taken into consideration when comparing unilateral versus bilateral TKR results as well as when planning a local arthroplasty service

To prevent the reported side effects of rhBMP-2, an important cytokine with bone forming capacity, the sustained release of rhBMP-2 is highly important. Synthetic copolymer polylactic acid-polyethylene glycol (PLA-PEG) is already shown to be a good carrier for rhBMP-2. The nano-sized hydroxyapatite (nHAp) is mentioned to be superior to conventional hydroxyapatite due to its decreased particle size which increases the surface area, so protein-cell adhesion and mechanical properties concomitantly. In the literature no study is reported with PLA-PEG / rhBMP-2/ nHAp for bone regeneration. In this study, we assessed the controlled release profile of rhBMP-2 from the novel biomaterial of PLA-PEG / rhBMP-2 / nHAp in vitro and evaluated the bone forming capacity of the composite in rat posterolateral spinal fusion (PSF) model in vivo. Composites were prepared via addition of rhBMP-2 (0µg, 3µg or 10µg) and nHAp (12.5mg) into PLA-PEG (5mg) + acetone solution and shaping. The release kinetics of the cytokine from the composites with 5µg BMP-2 was investigated by ELISA. The effect of nHAp and nHAp with rhBMP-2 on cell differentiation (rat BMSC cells, passage 3) was tested with ALP staining. In vivo bone formation was investigated by PSF on L4-L5 in a total of 36 male SD rats and weekly µCT results and histology at 8. th. weeks post operation were used for assessment of the bone formation. All animal experiments was approved by the institutional review board confirming to the laws and regulations of Japan. The composite showed an initial burst release in the first 24 hours (51.7% of the total released rhBMP-2), but the release was continued for the following 21 days. Thus, the sustained release of rhBMP-2 from the composite was verified. ALP staining results showed nHAp with rhBMP-2 contributed better on differentiation than nHAp itself. µCT and histology demonstrated that spinal fusion was achieved either one or both transverse processes in almost all BMP 3µg and BMP 10µg treated animals. On the contrary, only small or no bone formation was observed in the BMP0µg group (bilateral non-union / unilateral fusion/ bilateral fusion, BMP0µg group; 9/0/0, BMP3µg group; 1/0/11, BMP10µg group; 0/1/11). We developed a new technology for bone regeneration with BMP-2/PLA-PEG/nHAp composite. With this composite, the required dose of BMP-2 for spinal fusion in rats (10µg) was decreased to 1/3 (3µg) which can be explained by the superior properties of nano-sized hydroxyapatite and by the achievement of sustainable release of rhBMP-2 from the composite. An increase in metal ion levels is seen after implantation of all MoM hip prosthesis due to release from the surface directly, more so during articulation and corrosion of the bearing surfaces. The bearing surfaces in MoM prosthesis consist of cobalt, chromium and molybdenum. Several case-reports of cobalt toxicity due to a MoM prosthesis have been published in the last decade. Cobalt intoxication may lead to a variety of symptoms: neuro-ocular toxicity (tinnitus, vertigo, deafness, blindness, convulsions, headaches and peripheral neuropathy), cardiotoxicity and thyroid toxicity. Nausea, anorexia and unexplained weight loss have been described. Systemic effects from metal ions even with well functioning implants or with ion concentrations lower than those associated with known adverse effects may exist and warrant investigation. The aim of this study is to investigate self-reported systemic complaints in association with cobalt ion concentrations in patients with any type of MoM hip prosthesis. A cohort study was conducted. Patients with both unilateral and bilateral, resurfacing and large head metal on metal total hip arthroplasties were included for the current study. Blood metal ion concentrations (cobalt and chromium) were measured by inductively coupled plasma mass spectrometry (ICP-MS). Based on the known cobalt toxicity symptoms of case-reports and toxicology reports a new non-validated questionnaire was developed. questions were subdivided in general questions/symptoms, vestibular symptoms, neurological symptoms, emotional health and cardio- and thyroid toxicity symptoms. Independent samples T test, Fishers Exact Test and Pearsons (R) correlation were used. Analysis was performed on two groups; a low cobalt ion concentration group and a high cobalt ion concentration group A total of 62 patients, 36 (58%) men and 26 (42%) women, were included with a mean age at surgery of 60.8 ± 9.3 years (41.6 – 78.1) and a mean follow up of 6.3 ± 1.4years (3.7 – 9.6). In these patients a total of 71 prosthesis were implanted: 53 unilateral and 9 bilateral. Of these, 44 were resurfacing and 27 large head metal on metal (LHMoM) total hip arthroplasties. Mean cobalt and chromium ion concentrations were 104 ± 141 nmol/L (9 – 833) and 95 ± 130nmol/L (6 – 592), respectively. Based on the different thresholds (120 – 170 or 220 nmol/L) the low cobalt ion concentration group consisted of 44 (71%), 51 (82%) or 55 (89%) subjects respectively. No differences were found in general characteristics, independently of the threshold. The composite score of vestibular symptoms (vision, hearing, tinnitus, dizziness) was significantly higher (p < .050) in all high cobalt ion concentrations groups, independent of the threshold value This study aimed to detect a trend in self-reported systemic complaints in patients with metal-on-metal hip arthroplasty due to raised cobalt ion concentrations. Vestibular symptoms were more common in high cobalt ion concentration groups independent of the three threshold levels tested. The upper limit of acceptable cobalt ion concentrations remains uncertain. With regards to proactively inquired, self-reported symptoms the threshold where effects may be present could be lower than values currently applied in clinical follow-up. It is unknown what exposure to elevated metal ion concentrations for a longer period of time causes with aging subjects. Further research with a larger cohort and a more standardized questionnaire is necessary to detect previously undiscovered or under-reported effects

Although the impact of sexual difficulties on quality of life in patients with hip osteoarthritis has been documented in previous literature, recent research has shown that surgeons rarely discuss this sensitive topic with patients. The purpose of this study was to develop an educational tool to address common questions that patients may have regarding returning to sexual activity following their total hip arthroplasty (THA). The study was conducted in two phases. In Phase 1, patients who underwent a THA between 2013–2017 at a single centre were retrospectively identified and sent an anonymous online survey. This survey was aimed at assessing patient-specific concerns regarding whether they would have liked to receive information about returning to sexual activity, what information they would have liked to know and how they would have liked to receive this information. An educational tool was developed based on the findings of Phase 1. In Phase 2, prospective patients who were scheduled for a unilateral or bilateral THA were provided with the educational tool prior to their surgery. A questionnaire was administered to evaluate the effectiveness of this educational tool. Descriptive statistics and chi-squared tests were used for data analysis. In Phase 1, the overall response rate was 34.7% (n = 58/167). Out of the total respondents, 51.7% indicated an interest in receiving information on when to return to sexual activity following a THA. Patients selected an informational pamphlet as the most desired method of receiving information (p = .044). In Phase 2, the response rate was 54.5% (n = 30/55). Overall, 90% of patients felt that the pamphlet addressed all their concerns, and 93.3% felt they were provided with adequate information on how they could get more information. The pamphlet addressed questions regarding when it was safe to resume sexual activity following a THA, what positions were safe, and the associated risks. Individuals undergoing a THA are modestly interested in receiving information regarding when to return to sexual activity following their surgery, especially those who are sexually active preoperatively. This educational pamphlet may be useful in routine clinical practice in addressing concerns regarding returning to sexual activity. Understanding patients' goals and expectations for their postoperative course may help surgeons provide a more comprehensive approach to patient care

Aims

This study aimed to determine the expression and clinical significance of a cartilage protein, cartilage oligomeric matrix protein (COMP), in knee osteoarthritis (OA) patients.

Methods

A total of 270 knee OA patients and 93 healthy controls were recruited. COMP messenger RNA (mRNA) and protein levels in serum, synovial fluid, synovial tissue, and fibroblast-like synoviocytes (FLSs) of knee OA patients were determined using enzyme-linked immunosorbent assay, real-time polymerase chain reaction, and immunohistochemistry.

Purposes And Background. Having found a significant limitation of neural movement (66.6%) during SLR performed on the symptomatic side in patients with sub-acute lumbar intervertebral disc herniation (LIDH), we followed up on the same patients over 1.5 years to ascertain if changes in cord excursion accompany changes in clinical symptoms. Methods. 14 patients, who originally had sciatic symptoms due to subacute LIDH, were re-assessed both clinically and radiologically with a 1.5T magnetic resonance (MR) scanner. Displacement of the conus medullaris during the unilateral and bilateral SLR was quantified reliably with a randomized procedure and compared between maneuvers and with data from baseline. Multivariate regression models and backward variable selection method were employed to identify variables more strongly associated with decrease in low back pain and radicular symptoms. Results. Compared to previously presented baseline values, the data showed an extensive increase in neural sliding of 323.4% (2.52mm, p≤0.001) with the symptomatic SLR, 37.1% (0.82mm, p=0.0058) with asymptomatic SLR, and 48.2% (1.64mm, p≤0.001) with the bilateral SLR. Increase in neural sliding correlated significantly with decrease of both radicular symptoms (Pearson=−0.719, p≤0.001) and low back pain (Pearson=−0.693, p≤0.001). Multivariate regression models and backward variable selection method confirmed that improvement of neural sliding effects (p≤0.004) as the main variable being associated with improvement of self-reported clinical symptoms. Conclusion. To our knowledge, these are the first non-invasive data to objectively support the association between increase in magnitude of neural adaptive movement and decrease in clinical symptoms in in-vivo and structurally intact human subjects. Aim. The purpose of this study was to analyze the patellofemoral kinematics in neglected adult developmental dysplasia of the hip patients with patellofemoral symptoms and tried to clarify the affect of the severity of dislocation, the amount of limb length discrepancy, the deviation of mechanical axis and the changes in femoral anteversion on patellofemoral alignment. Methods. The dynamic patellofemoral CT results of 39 patients with DDH suffering from knee pain were reviewed. The mean age was 40.07 (range: 22–61). 14 of them were bilateral and 25 were unilateral neglected DDH patients. The CT results of 12 patients suffering from unilateral patellofemoral pain following the treatment of locked intramedullary nailing was taken as control group. In this patients atraumatic and asymptomatic normal site was taken as control group. Results. In unilateral neglected DDH patients there was significantly higher medial patellar displacement in 0, 15, 30, 60 degrees flexion in the knee at the site of dislocation. Again in uninvolved site medial patellar displacement in 15, 30, 60 degrees flexion was higher with respect to control group. In the involved extremity the PTA angle in 0, 15, 30, 60 degrees flexion were significantly higher than in control group. This increase in PTA angle corresponding to medial patellar tilt was observed only in involved extremity. In the knees of patients with bilateral DDH there was significant medial patellar displacement in every flexion degrees with respect to control group. Besides in bilateral DDH patients, the PTA angle in 15, 30, 60 degrees flexion were significantly higher than control group corresponding to medial patellar tilt. The amount of leg length discrepancy and the severity of dislocation as well as mechanical axis deviation were not affecting the patellofemoral parameters in both unilateral and bilateral DDH patients. Conclusion. Both in unilateral and bilateral DDH patients there are major changes in patellar tracking on femur during knee flexion. Increased medial shift and medial patellar tilt were seen in these patient groups. The neglected DDH patients suffering from knee pain should be analyzed not only for tibiofemoral abnormalities but also for patellofemoral malignment

Aims

The first metatarsal pronation deformity of hallux valgus feet is widely recognized. However, its assessment relies mostly on 3D standing CT scans. Two radiological signs, the first metatarsal round head (RH) and inferior tuberosity position (ITP), have been described, but are seldom used to aid in diagnosis. This study was undertaken to determine the reliability and validity of these two signs for a more convenient and affordable preoperative assessment and postoperative comparison.

Methods

A total of 200 feet were randomly selected from the radiograph archives of a foot and ankle clinic. An anteroposterior view of both feet was taken while standing on the same x-ray platform. The intermetatarsal angle (IMA), metatarsophalangeal angle (MPA), medial sesamoid position, RH, and ITP signs were assessed for statistical analysis.
