The accessory navicular (AN) is a separate ossification center for the tuberosity of the navicular that is present in approximately 5–14% of the general population. It produces a firm prominence on the plantar-medial aspect of the midfoot. There may be a co-existent flexible flatfoot, but there is no conclusive evidence of a cause-and-effect relationship between the two conditions. It is usually not symptomatic, and few cases necessitate operative intervention. When symptoms require surgical treatment, excision of the AN, with or without advancement of the posterior tibial tendon, usually is considered. To describe new technique of AN excision and tibialis posterior tendon advancement (TPTA) using a bio-absorbable tenodesis screw and to investigate the outcome of this cohort in comparison to conventional simple excision. Retrospective 2 Cohort study. Single surgeon series from single institution. All patients younger than 18 years from Jan 2000 to Aug 2012 undergoing simple excision (SE) or excision with TPTA were identified from the prospectively collected database. Case notes were reviewed and data regarding demographics, indications for surgery, presence of Pes Planus, time from presentation to surgery, length of follow-up, patient satisfaction and complications were recorded. Pain and functional outcome were measured using Visual Analogue Score (VAS) and patient reported outcome measure- Oxford Ankle Foot Questionnaire- Child and Teenager Version. There were 6 patients in SE group. There were 3 males and 3 females. Mean age at surgery was 13.9 years. Surgical indication was painful swelling in all patients and in addition 1 had pes planus. There were 7 in SE & TPTA group. There were 3 males and 4 females. The mean age at surgery was 13.1 years. Surgical indication was painful swelling in all patients and in addition 1 had hind foot rigidity and 3 had pes planus. Postoperative protocol involved weight bearing with or without cast in SE group and non-weight bearing in cast for 6 weeks in TPA group. All patients reported excellent to good outcome. There were no complications and no reoperations after tendon advancement. In conclusion, based on our study findings, we think AN excision and TP advancement is a safe and effective technique for symptomatic pain relief. It is a novel technique which achieved excellent to good outcome in our series

The purpose of this study was to determine the surgical risks and recurrence rate associated with the excision of osteochondroma from the long bones most frequently operated on in our institution; the femur, tibia, humerus and fibula. Two hundred and twenty four osteochondromata were excised in total between July 1992 and January 2001. The medical records and radiographs of 126 patients who had 147 osteochondromata excised from the femur, tibia, humerus and fibula were reviewed. Of these, 30 patients presented with multiple osteochondromata, accounting for 48 of the 147. Fifty three involved the femur (2 proximal), 55 the tibia (16 distal), 12 the fibula (2 distal) and 27 the proximal humerus. The mean age at excision was 12.5 years (2–18 years) and the mean follow-up was five years (1 to 10 years). There were 15 surgical complications (10% of excisions) including one compartment syndrome, five superficial wound infections, two haematoma formations which required evacuation, one partial wound dehiscence, one deep infection with sinus formation which required excision, one sural nerve and one saphenous nerve neuropraxia, one cutaneous nerve entrapment and two hypertophic scar/keloid formations. The patient with the compartment syndrome had excision of a distal femoral, proximal tibial and fibular osteochondroma during the same procedure and was diagnosed to have won Willebrand disease after the surgery. There were eight recurrences involving five patients with multiple osteochondromata and three in whom the excision was incomplete due to the proximity to neurovascular structures. Surgical risks related to excision of osteochondroma are relatively frequent and must not be underestimated. Excision should therefore only be performed if strongly indicated. The recurrence rate (5.5%) seems to be higher than previously reported in the literature (2%) and generally affects patients with multiple osteochondromata. Incomplete excision resulted in recurrence in all our cases

Patient reported outcomes and satisfaction scores following excision of interdigital Morton's neuroma have been recently established. However, little is known regarding what patient factors affect these outcomes. This is the first and largest prospective study to determine which patient factors influence surgical outcome following Morton's neuroma excision. Over a seven-year period, 99 consecutive patients (112 feet) undergoing surgical excision of Morton's neuroma were prospectively studied. 78 patients were female with a mean age at operation of 56 years. Patient recorded outcomes and satisfaction were measured using the Manchester-Oxford Foot Questionnaire (MOXFQ), Short Form-12 (SF12) and a supplementary patient satisfaction survey three months pre and six months post-operatively. Patient demographics were recorded in addition to co-morbidities, deprivation, associated neuroma excision and other forefoot surgery. Obesity, deprivation and revision surgery proved to statistically worsen MOXFQ outcomes post-operatively (p=0.005, p=0.002 and p=0.004 respectively). Deprivation significantly worsened the mental component of the SF12 (p=0.043) and depression the physical component (p=0.026). No difference in outcome was identified for age, sex, time from diagnosis to surgery, multiple neuroma excision and other forefoot surgeries. 23.5percnt; of deprived patients were dissatisfied with their surgery compared to 7percnt; of the remaining cohort. Patient reported outcomes following resection of symptomatic Morton's neuroma are shown to be less favourable in those patients who display characteristics of obesity, depression, deprivation and in those who undertake revision neuroma resection. Surgery can be safely delayed, as time to surgery from diagnosis bears no impact on clinical outcome

Introduction. The popliteus tendon is a component of the posterolateral corner of the knee, which controls the external rotation of the tibia. In our clinical practice, the femoral footprint of the popliteus tendon is occasionally excised as the bone is resected during total knee arthroplasty (TKA). Although the excision of the popliteus tendon femoral footprint could result in excessive external rotation of the tibia and may have adverse effects on the long-term outcomes of TKA, little attention has been paid to the popliteus tendon femoral footprint during TKA. The purpose of the present study is to assess the frequency of the excision and its associated risk factors. Methods. One hundred eleven knees of 90 patients with varus knee osteoarthritis who underwent primary TKA were included in the present study. There were 13 males and 77 females, and their average age was 74 years. The NexGen knee replacement system (Zimmer, Warsaw, IN, USA) was used in all cases. The excision of the popliteus tendon femoral footprint was intraoperatively evaluated, and the patients were divided into three groups depending on the status of the femoral footprint, i.e., the preserved, partially excised, and completely excised groups. The thickness of the distal femoral osteotomy, femoral component size, and background data including height, body weight, gender, and age were compared among these groups. Analysis of variance followed by Student–Newman–Keuls test were used to compare the continuous values and ordinal scales. Gender was compared using Fisher's exact test and residual analysis. Statistical significance was set at p < 0.05. Results. The popliteus tendon femoral footprint was preserved in 48 knees (43.2%), partially excised in 45 knees (40.5%), and completely excised in 18 knees (16.2%). The mean patient height was 154.6, 150.1, and 148.7 cm in the preserved, partially excised, and completely excised groups, respectively, and these differences were statistically significant (p < 0.01). Femoral component size was significantly smaller in the partially and completely excised groups compared with that in the preserved group (p < 0.05). The preserved group included more male patients (p < 0.01). There were no significant differences in body weight, age, and thickness of the distal femoral osteotomy among the groups. Conclusion. The partial or complete excision of the popliteus tendon femoral footprint was observed in more than half of the evaluated knees. Shorter height, smaller femoral component size, and female sex were considered to be the possible risk factors for the excision of the popliteus tendon femoral footprint

Chondrosarcoma of bone is a surgical disease and excision with wide margins is the optimum treatment. Sometimes the size or location of the tumour at the time of diagnosis mean that only a marginal excision can be achieved. The effect of the margin of excision on outcome is investigated. Method. All patients with newly diagnosed primary chondrosarcoma of bone and without metastases at the time of diagnosis were identified from a prospective database. Their outcome was investigated to assess whether the margin of excision affected outcome. Results. 492 patients were included in the analysis with a mean age of 48. The mean tumour size was 11cm and 59% were male. The 10 year tumour specific survival was 85% for clear cell (N=7) and grade 1 (N=210), 60% for grade 2 (N=180), 47% for grade 3 (N=59) and 16% for dedifferentiated (N=36). The 10 year local recurrence free survival was 86% (clear cell), 73% (grade 1), 67% (grade 2), 36% (grade 3) and 56% dedifferentiated. Local recurrence was strongly related to older age (p=0.0065), grade (p⋋0.0001) and margins (p⋋0.0001). Patients who developed local recurrence had a 43% survival at 10 years compared with 76% for those who did not (p⋋0.0001). They also had a 49% risk of developing metastases compared to a 17% risk for those without local recurrence (p⋋0.0001). However most of these patients had metastases before developing LR(57%). 16 of 65 patients with local recurrence but no metastases, died due to local progression (usually pelvic or spinal tumours). There was however no relation of the margin of excision to either the risk of developing metastases or survival for any grade of tumour. Conclusion. Whilst local control is strongly related to margins of excision and grade, overall survival is related principally to grade but not to margins

Wrist ganglia are the commonest benign tumours of the hand consisting of a collagenous walled cavity containing gelatinous mucin material. These lesions can be managed by either reassurance alone, aspiration or surgical excision. However studies evaluating patient outcomes following these treatment modalities are limited and between them have often presented inconsistent results. Some recently published data has suggested that there is no long-term benefit of excision or aspiration over reassurance alone and as a result surgical excision of wrist ganglia has fallen out of favour with some health care trusts. This aim of this retrospective, questionnaire based study was to assess patient outcomes following wrist ganglion excision surgery and to compare these results with current published evidence. Sixty two patients were identified from the hospital records database between July 2003 and March 2008. Fifty patients (80.6%) responded to a questionnaire by post or telephone call with a mean time to follow up of thirty nine months (range 16 to 71 months). Pain and cosmetic concern were the primary symptoms preoperatively, experienced by 78% and 70% of patients respectively. 26% to 48% of patients experienced other symptoms such as pins and needles, numbness, weakness and stiffness. Following surgery, there was a statistically significant reduction in all symptoms experienced. Ganglions recurred at the same location in five patients (12%) of which four would consider further surgery. 96% of patients were satisfied with the treatment they received. Our results regarding symptomatic relief are comparable with current published data. In addition our data provides further evidence that ganglion excision surgery prevents recurrence to a greater extent than either aspiration or reassurance alone. Furthermore our recurrence rate is lower than other published reports evaluating ganglion excision surgery which we believe underestimate the benefits offered by this treatment modality. As a result, wrist ganglion excision surgery remains an important treatment modality offering excellent results in terms of symptom resolution, patient satisfaction and ganglion recurrence

Twenty-two patients who underwent thirty-four Kellers’ excision arthroplasty were followed up at an average of thirty-five months. They were assessed using AFAOS, satisfaction and radiological evaluation. The average hallux score was eighty- five (fifty-two to one hundred) while the average lesser toe score was ninety- two (seventy-five to one hundred). The average pain score was thirty- six (twenty to forty) for the hallux and thirty-eight for the lesser toes (twenty to forty). 23/34(68%) had good to excellent, 6/34 (18%) had fair and 5/34(14%) had poor results. The great toe was moderately short, but most patients do not seem to mind this. 91% patients were satisfied with the results. We undertook a retrospective study of Kellers’ excision arthroplasty done over the last seven years to assess the medium term results. Twenty-two patients who underwent thirty- four Kellers’ excision arthroplasty were followed up at an average of thirty-five months. They were followed up using the AFAOS, patient satisfaction and radiological evaluation. The average age at the time of surgery was 67.4 years. There were seventeen females (twenty-five feet) and five males (nine feet). All patients underwent bunionectomy along with excision of proximal third of the proximal phalanx. Of these twenty underwent K wire stablization of the hallux following excision. The average hallux score was eighty- five (range sixty-two to one hundred) while the average lesser toe score was ninety- two (range seventy-five to one hundred). The average pain score was thirty- six (range twenty to forty) for the hallux and thirty-eight for the lesser toes (twenty to forty). 23/34 (68%) had good to excellent, 6/34 (18%) had fair and 5/34(14%) had poor results. The average correction of the hallux valgus was 9o. The average IMT was 25o preoperatively and 18o postoperatively. The average shortening was 7 mm. Complications included two cases of transfer metatarsalgia and two cases of clawing of the lesser toes. One patient developed abscess which settled after incision and drainage. Thirty-one out of thirty-four patients were satis-fied with the final outcome and thirty out of thirty-four patients would have the operation on the other feet. The great toe is moderately short, but most patients do not seem to mind this. 91% patients were satisfied with the results

Objective: To assess whether hemicortical excision and biological reconstruction instead of the conventional wide resection for selected cases of malignant surface tumors, may give adequate oncologic clearance with less morbidity and better function. Methods: From January 2000 to June 2007 a total of 29 malignant surface tumors were managed at our institute. Fifteen of these were treated with hemicortical excision and reconstruction. They consisted of 10 parosteal osteosarcomas, 1 periosteal osteosarcoma, 1 high grade surface osteosarcoma, 1 adamantinoma and 2 surface chondrosarcomas. Nine were in the distal femur, 3 in tibia, 2 in the humerus and 1 in the radius. Four of these were residual/recurrent lesions following earlier intervention. After hemicortical excision with adequate margins the bone defect was reconstructed with allograft/autograft and suitable internal fixation where indicated. Results: Margins were reported free in all cases. There were no infections. Three of five patients who did not have fixation with a plate at index surgery sustained a subsequent fracture which was then reduced and fixed. Follow up duration was from 24 to 90 months. Two patients had isolated soft tissue recurrences. Both underwent re excision and are currently disease free at 36 and 38 months respectively after recurrence. Eleven patients had a maximum possible score of 30 (MSTS functional scoring); other four had a score of 29. None of the cases have developed distant metastasis. Discussion: Early results indicate that in selected cases, hemicortical excision is an oncologically sound procedure. The ability to do a biological reconstruction with bone helps avoid some of the serious complications of a megaprosthetic reconstruction while permitting full active loading of the extremity and near normal function. Internal fixation at index surgery permits early mobilization and minimises the incidence of subsequent fractures

The recognition, investigation and management of osteoid osteoma has been well documented. Treatment can either be medical or surgical, studies have shown both options to have almost equal long term outcomes. However only surgical treatment involving complete excision of the lesion allows for confirmatory tissue diagnosis of osteoid osteoma especially important in cases where symptoms and signs are atypical. Several methods of surgical treatment with varying degrees of success have been described in literature. Here we describe a surgical techniques for the treatment of osteoid osteoma which enables removal of the lesion in a precise manner using a precision bone graft trephine with minimal excision of bone. This technique will be very useful in the excision of lesions in areas in which excessive excision of bone can lead to an unstable bony structure which can predispose to fracture. To our knowledge this surgical technique in the management of osteoid osteoma has not been described in the literature

Routinely in TKA, at least one of the cruciate ligaments are sacrificed. The cruciate ligaments excision may have an impact in the stability of the reconstructed knee by virtue of the impact on the gap kinematics. In this study, a selective cutting protocol was designed to quantify the individual contribution of ACL and PCL about the knee by means of a loaded cadaveric model. Five fresh frozen normal cadaver specimens were used. The femur was fixed to a specially designed machine, and 3D tibial movements relative to the femur and joint gap distances were measured by means of a navigation system from full extension to 140° flexion. The joint was distracted with 10 pounds. The measurement was performed before and after ACL and PCL excision. Medial gap distance at 90° flexion before and after cruciate ligaments excision was 4.3 ± 2.7 mm (mean ± SD) and 5.1 ± 2.8 mm (p< 0.05) respectively. Cruciate ligaments excision significantly widened the medial and lateral gaps at many flexion angles, and the effect of excision on the gap distance was different between medial and lateral sides especially at 90° knee flexion. Cruciate ligaments excision also significantly influenced knee kinematics. If this varying gap is not accounted for either through implant shape and orientation or through soft tissue adjustments, instability could be the result. Surgeons should be made aware of the influence of cruciate excision on varus/valgus laxity throughout the range of motion. Design modification of the femoral component may also be necessary in order to obtain optimal stability in deep flexion

Introduction and Aims: The volar carpal ganglion (VCG) is a common benign tumor of the hand complicated by multiple recurrences with conventional treatment. Despite being a common condition, we know of no report describing post-operative functional outcome. We evaluated functional outcome, pain, scarring, and recurrence after VCG excision in the last 15 years. Method: Twenty-eight patients were considered for inclusion in this study. All patients had been surgically treated for volar carpal ganglia in the last 15 years. Of these, 16 were available for follow-up after exclusion of patients who had since deceased, moved, or were otherwise lost to follow-up. Mean follow-up period was 55 months (nine–135 months). Functional outcome of the upper extremity was evaluated using the standard DASH questionnaire. DASH scores can range from zero (no difficulty in performing daily tasks) to 100 (unable to perform daily tasks). Pain, scarring, and recurrence were also assessed. Results: Sixteen patients who underwent surgical excision of their recurrent volar carpal ganglions were available for follow-up at the time of the study. They included six men and 10 women, with a mean age of 47 (range 11–70 years). The duration of symptoms prior to excision ranged from two to 50 months. Pain was the main reason for surgery while one patient reported that surgery was performed for cosmetic reasons. There were eight patients in the nine-month to three-year follow-up period and the average DASH score was 7.6. The four patients in the three to five-year follow-up period had an average DASH of 8.1. The four patients in the five plus years of follow-up had an average DASH of 1.7. The improvement in functional outcome correlated with time since surgery, however these differences were not statistically significant. Two of the 16 patients had recurrences within six months after excision. Increased pain was reported in these patients only. No patients complained of significant scarring post-operatively. Conclusion: Multiple treatment modalities including aspiration and surgery are discussed in the literature with great outcome variability. Our series is the first long-term functional assessment after VCG excision. Thus, surgery is an option for treatment of this common condition as evidenced by low recurrence and DASH scores and high patient satisfaction

Introduction Radio-scapho-lunate (RSL) arthrodesis has been shown to be an effective treatment for arthritis limited to the radio-carpal joint. It preserves wrist motion at the mid-carpal joint while relieving pain. The main shortcoming of this procedure has been restricted residual wrist range of motion (ROM) compromising clinical outcome. The aim of the study was to assess the effect of excision of distal scaphoid and triquetrum on wrist motion following RSL arthrodesis. Methods Ten cadaveric wrists had their range of motion measured before and after RSL arthrodesis and after sequential distal scaphoid and then triquetral resection. The mean and standard deviation of the change in motion were calculated for each step. The two-tailed Student’s t-test with p < 0.05 was used to determine the statistical significance of the changes. Results Distal scaphoid excision after RSL arthrodesis resulted in 25° (35%, p< 0.01) increase in flexion-extension (F-E) arc and 11° (34%, p< 0.01) increase in radio-ulnar (R-U) arc. Subsequent excision of triquetrum further increased F-E arc by 13° (13%, p< 0.05) and R-U arc by 9° (21%, p< 0.01). Conclusions In the cadaveric wrists, distal scaphoid excision resulted in significantly improved R-U arc and F-E arc. Subsequent triquetral excision further improved wrist ROM. Modification of RSL fusion to include distal scaphoid and triquetrum excision should be considered to improve residual wrist motion

Background:. Achilles pathology is a serious and frequently occurring problem, especially in elite athletes. Recent research has suggested a role for the plantaris tendon in non-insertional achilles tendinopathy. We report on the outcomes after excision of the plantaris tendon in elite athletes. Aim:. To assess whether or not excising the plantaris tendon improves the symptoms of Achilles tendinopathy in elite athletes. Methods:. A group of 32 elite athletes who underwent plantaris tendon excision to treat medially located pain associated with non-insertional Achilles tendinopathy were investigated. Outcomes were assessed with pre and post-operative Visual Analogue Scores (VAS) for pain and the Foot and Ankle Outcome Score (FAOS) as well as time to return to sport and satisfaction scores. Results:. At a mean follow-up of 22.4 months (12–48), 29/32 (90%) of athletes were satisfied with the results. 30/32 athletes (94%) returned to sport at a mean of 10.3 weeks (5–27). The mean VAS score improved from 5.8 to 0.8 (p<0.01) and the mean FAOS improved in all domains (p<0.01). Conclusions:. The plantaris tendon may be responsible for symptoms in some patients with non-insertional Achilles tendinopathy. Excision using a mini-incision technique carries a low risk of complications and may provide significant improvement in symptoms enabling an early return to elite level sports

Introduction: This biomechanical study evaluates the consequences of a mid-third BPTB-autograft excision on patellofemoral biomechanics and knee kinematics. Of particular interest was the potential role of a BPTB-autograft excision on postoperative anterior knee pain in ACL replacement surgery. Methods: Isokinetic knee extension from 120 of flexion to full extension was simulated on 9 human knee cadaver specimens (5 male, 4 female, average age at death 43 years). Joint kinematics was evaluated by ultrasound sensors (CMS 100TM, Zebris, Isny, Germany), and retro-patellar contact pressure was measured using a thin-film resistive ink pressure system (K-ScanTM 4000, Tekscan, Boston). All data were taken before and after excision of a mid-third BPTB-autograft. Results: Following excision of a mid-third patella tendon autograft we found a significant (p< 0.05) proximalization of the patella (average: 0.5 mm) and a significant decrease of patella flexion in the sagittal plane (average: 1). Patella tilt, -rotation (frontal plane), -translation (medial/lateral) and tibiarotation (external-/internal), -axis (varus-/valgus position) remained unchanged. Patellofemoral contact pressure and -area decreased significantly near knee extension (p< 0.05). Conclusions: We conclude that an excision of a mid-third patella tendon autograft results in a lengthening of the tendon with a proximalization of the patella. As the patellofemoral pressure decreases and the patella remains centralized, postoperative anterior knee pain following ACL-replacement using a BPTB autograft can not be explained by the results of our study

Purpose: Patients are often referred to tertiary care centers after unplanned excision of soft tissue sarcomas. In situations where the tumour is small and superficial, the situation can often easily be salvaged by re-excision of the tumour bed. However, if the original tumour is large, deep to fascia or directly adjacent to bone or neurovascular structures, the salvage procedure often becomes more complex and morbid. The purpose of this study is to evaluate the effect of unplanned excision of “high-risk” soft tissue sarcomas on patient outcome. Method: We reviewed our prospectively collected sarcoma database from 1989 to 2006. Patients who underwent definitive resection of a soft tissue sarcoma at our centre were included. Patients were divided into 2 groups based on whether or not they had undergone initial unplanned resection of their tumour prior to referral to our centre for definitive management. Low risk patients who had tumours that were less than 5 cm in diameter, superficial to fascia, and not overlying bone or neurovascular structures were excluded. Results: A total of 1034 patients met inclusion criteria. Of these, 385 (37%) patients had undergone an unplanned excision prior to referral, while 649 (63%) patients were referred to our centre with an intact tumour without prior unplanned excision. There was a higher percentage of high grade (61% vs. 50%) and deep tumours (88% vs. 65%) in the unplanned excision group, but the mean tumour diameter was smaller in the unplanned excision group (5.9 cm) compared to the control group (10.6 cm). There was no difference between the groups in terms of rate of amputation, necessity for flaps for coverage, and local recurrence-free survival. Complications were more common in the control group (34%) than the unplanned excision group (20%, p< 0.0005, Chi-square). 5-year overall (p< 0.00005, log rank) and metastasis-free (p< 0.00005, log rank) survival were higher in the unplanned excision group. There was no difference in TESS, MSTS87 or MSTS93 functional outcome scores between the groups. Conclusion: Patients referred to a tertiary sarcoma centre after unplanned resection of a soft tissue sarcoma can still be salvaged with appropriate multidisciplinary care. Patients referred after unplanned excision appear to fare at least as well as those initially resected at our centre, and fare better in terms of some outcomes studied. The majority of this difference can likely be explained by significant differences in important prognostic factors such as grade, size and depth between the unplanned excision group and control group initially managed at our centre. Patients referred after unplanned excisions appear not to have as dismal an outcome as has been previously reported

Objectives. The purpose of this study was to examine the effect of posterior cruciate ligament (PCL) retention, PCL recession, and PCL excision during cruciate-retaining total knee replacement. Methods. A total of 3018 anatomic graduated component total knee replacements were examined; 1846 of these retained the PCL, 455 PCLs were partially recessed, and in 717 the PCL was completely excised from the back of the tibia. Results. Clinical scores between PCL groups favored excision for flexion (p < 0.0001), and recession and retention for stairs (p < 0.0001). There was a mild difference in long-term all-cause aseptic survivorship between PCL-retained (96.4% at 15 years) combined with PCL-recessed groups (96.6% at 15 years) when compared with the PCL-excised group (95.0% at 15 years) (p = 0.0411, Wilcoxon; p = 0.0042, log-rank), as well as tibial or femoral loosening, which reported prosthesis survival of 97.8% at 15 years for PCL-retained knees, 98.2% for recessed knees, and 96.4% for excised knees (p = 0.0934, Wilcoxon; p = 0.0202, log-rank). Conclusions. Despite some trade-off in clinical performance, if the PCL is detached at the time of operation, conversion to a posterior-stabilised prosthesis may not be necessarily required as long as stability in the anteroposterior and coronal planes is achieved

Limited wrist arthrodesis has been shown to be an effective treatment for the degenerative and unstable wrist, abolishing pain but limiting motion. The aim of the study was to assess the effect of excision of the scaphoid and triquetrum on wrist joint range of motion, in the setting of a limited midcarpal arthrodesis. Twelve cadaveric wrists had the range of motion measured, before and after, ulnar four-corner fusion (lunate, capitate, triquetrum and hamate fusion). This was measured again following sequential scaphoid and triquetral resection. Scaphoid excision after four-corner arthrodesis resulted in a 12 degrees increase in the radio-ulnar (R-U) arc and 10 degrees increase in the flexion-extension (F-E) arc range of motion. Subsequent excision of the triquetrum, to produce a three-corner fusion, further increased R-U arc by seven degrees and F-E arc by six degrees. These results demonstrate that three-corner fusion with excision of scaphoid and triquetrum results in improvement in wrist motion when compared to four-corner fusion with scaphoid excision alone. From this we conclude that triquetrum excision should be considered in Scapholunate advanced collapse (SLAC) wrist reconstruction to improve residual wrist range of motion

Aim: A prospective study designed to assess and evaluate the results of arthroscopic acromioclavicular joint (ACJ) minimal excision arthroplasty. Patients and Methods: Twenty-two patients with ACJ arthropathy underwent an arthroscopic limited excision of the ACJ preserving the superior ligamentous complex. The patients were assessed pre and post operatively using the Constant score. The average time from surgery to review was five months (three to nineteen months). A self assessment questionnaire was also used, evaluating outcome measures such as activities of daily living, shoulder function and patients satisfaction. Results: The mean preoperative Constant score was 28 and postoperative score was 71 with an improvement in pain from 15% to 80% and the range of motion from 37% to 84%. The preoperative self assessment score was 25.9 and postoperative score was 83 with an improvement in activities of daily living from 28% to 79%, and shoulder function from 34% to 87%.Twenty patients were very satisfied, one was moderately satisfied and one patient was dissatisfied although his shoulder function increased from 20 to 60%. Discussion: This study suggests that this technique is well accepted by the patients and results in a good improvement in shoulder function. The results are comparable with those reported in the literature concerning the standard ACJ arthroplasty. We therefore believe that a limited excision of the ACJ is adequate and a reliable alternative to conventional techniques

Fibular hemimelia is associated with equinovalgus deformity of the ankle and hind foot and antero-medial bowing of tibia. A wedge shaped distal epiphysis of the tibia and tight posterolateral soft tissues play an important role in the pathogenesis of ankle valgus and lateral subluxation of foot. Tethering effect of fibular anlage may contribute to the deformities in the tibia and ankle. Lengthening procedures are associated with progression of these deformities. The purpose of this study is to determine whether Exner Osteotomy and Excision of Fibular anlage will correct the valgus deformity of the ankle and antero-medial bowing of tibia. A bending osteotomy through the distal tibial physis as described by Exner and excision of Fibular Anlage was performed in six limbs in five children (4 boys, 1 girl) with fibular hemimelia. Histology of Excised Fibular anlage was studied under light microscopy. The mean age at the time of surgery was twenty two months (range: 8 months to 5 years). The mean follow-up was two years and two months (13 months to 4 years and 8 months). Full Correction of ankle valgus and tibial bowing was achieved in three feet where, a cortical strut graft was used in the open wedge osteotomy. In two feet synthetic bone substitute was used. In these, tibial bowing corrected but slight ankle valgus remained. In one foot where synthetic bone substitute was used and the postoperative compliance with AFO was poor, bowing of tibia improved but ankle valgus recurred. Premature fusion of growth plate did not occur in any of the cases. Histology of fibular anlage showed replacement of bone tissue by mature collagen bundles surrounded by fibroconnective tissue. Exner Osteotomy and Excision of Fibular Anlage in Fibular Hemimelia corrects the ankle valgus and antero-medial bowing of tibia

Failures of treatment of osteoid osteoma (OO) are related to errors in exact localization and incomplete excision of the nidus. Intraoperative Iso-C 3D navigation allows exact localization, excision and confirmation of excision by percutaneous methods. We report the successful percutaneous excision of OO in 11 patients (extremities-5; spine-6). All patients had a minimally invasive reflective array (MIRA) fixed to the same bone in the extremities and to the adjacent spinous process or body(caudal) in spine, followed by registration of anatomy. A tool navigator was utilized to plan the key hole incision so that the trajectory did not involve important anatomical structure. A sleeve was then introduced which allowed the usage of instruments like a burr and curette to deroof the nidus, curette the nidus and obtain material for histopathology and further burr the cavity to ensure complete eradication of the nidus. During the entire procedure, the tool navigator was used frequently to reconfirm the location and the depth of burring. Following excision, registration using Iso-C 3D C-arm was done to confirm the complete eradication of the nidus. The age of the patients varied from 10 years to 27 years. In the extremities, location of the MIRA was in the same bone and firm anchorage was obtained using either a single Steinman pin locator (4 patients) or a double pin locator (1 patient). In spine the MIRA was attached to the adjacent spinous process (caudal) in the cervical, thoracic or lumbar region (5 patients) and in sacrum (1 patient) it was attached using a Steinman pin to the adjacent vertebral body. Excellent three-dimensional view of the nidus and localization was possible in all patients. A safe trajectory that avoided anatomical structures was possible in all patients using a tool navigator. The incision ranged from 1 to 4 cms. Adequate material for histology was obtained in ten patients that confirmed the diagnosis of osteoid osteoma and in one patient histopathological confirmation was not possible because the nidus was completely destroyed during the process of deroofing and burring. In ten patients, post excision ISO-C 3D scans confirmed adequate removal and in one patient, it was successful in identifying incomplete removal requiring further excision of the nidus. The average operating time was 62 mins (37–90 mins) and the blood loss was less than 30 cc in all patients. All patients achieved excellent pain relief and were asymptomatic at an average follow up of 3.4 (2.2 – 3.9) years. Iso-C 3 D navigation offers the advantage of excellent localization of the nidus and percutaneous excision of these tumors, thereby conserving bone in critical locations like the spine and upper end of femur. It also offers the advantage of intraoperative confirmation of adequate excision and allows harvesting the nidus for histological confirmation
