Methods
The foot and ankle are very commonly affected in various paralytic conditions. Paralysis of different muscles acting on the foot results in characteristic gait aberrations. The gait abnormalities are a result of one or more of the consequences of paralysis including: loss of function, muscle imbalance, deformity and instability of joints. The aims of treatment of the paralysed foot and ankle are to: make the foot plantigrade, restore active dorsiflexion during the swing phase of gait (if this is not possible then prevent the foot from ‘dropping’ into plantar flexion during swing), ensure that the ankle and subtalar joints are stable throughout the stance phase of gait, facilitate a powerful push-off at the terminal part of the stance phase (if this is not possible, at least prevent a calcaneal hitch in terminal stance). The specific aims of treatment in each patient depend on the pattern and the severity of paralysis that is present and hence the aims are likely to vary. In order to determine what treatment options are available in a particular patient, it is imperative that a careful clinical assessment of the foot is done. Based on the clinical assessment, these questions need to be answered before planning treatment: What are the muscles that are paralysed What is the power of each muscle that is functioning? Is there muscle imbalance at the ankle, subtalar or midtarsal joints that has either already produced a deformity or has the potential to produce a deformity in future? Are there any muscles of grade V power that can be spared for a tendon transfer without producing a fresh imbalance or instability. To facilitate responses to these questions, the muscle power of each muscle can be charted on a template that facilitates graphic representation of the muscle balance around the axes of the ankle and subtalar joints. This assessment clarifies whether a tendon transfer is a feasible option. If a tendon transfer is considered feasible, then the following questions also need to be answered: Is there a fixed, static deformity that needs to be corrected prior to a tendon transfer? If a tendon transfer was performed, would the child be capable of comprehending and cooperating with the post-operative muscle re-education programme?. The

Increasingly often diagnosed by sophisticated investigation, rotator cuff disease may be treated conservatively or surgically. Surgical options are decompression alone, decompression and debridement, partial rotator cuff repair, full reconstruction, tendon transfer and prosthetic replacement. Emphasising the quality of the repair rather than the classification of the injury, this paper details the criteria used in reaching a decision about the type of surgical treatment. Clinical, radiological, ultrasonographic and arthroscopic findings are used. The prognosis is directly related to the repair.
Dissatisfaction rates after TKA are reported to be between 15 – 25%, with unmet outcome expectations being a key contributor. Shared decision making tools (SDMT) are designed to align a patient's and surgeon's expectations. This study demonstrates clinical validation of a patient specific shared decision making tool. Patient reported outcome measures (PROMs) were collected in 150 patients in a pre-consultation environment of one surgeon. The data was processed into a probabilistic predictive model utilising prior data to generate a preoperative baseline and an expected outcome after TKA. The surgeon was blinded to the prediction algorithm for the first 75 patients and exposed for the following 75 patients. PROMs collected were the knee injury and osteoarthritis outcome score (KOOS) and questions on lower back pain, hip pain and falls. The patients booked and not booked before and after exposure to the prediction were collected. The clinical validation involved 27 patients who had their outcome predicted and had their PROMs captured at 12 months after TKA. The predicted change in severity of pain and the patients actual change from pre-op to 12 month post operative KOOS pain was analysed using a Spearman's Rho correlation. Further analysis was performed by dividing the group into those predicted by the model to have improved by more than 10 percentile points and those who were predicted to improve by less than 10 percentile points.Introduction
Methods
Although wait-times for hip fracture surgery have been linked to mortality and are being used as quality-of-care indicators worldwide, controversy exists about the duration of the wait that leads to complications. Our objective was to use new population-based wait-time data to emprically derive an optimal time window in which to conduct hip fracture surgery before the risk of complications increases. We used health administrative data from Ontario, Canada to identify hip fracture patients between 2009 and 2014. The main exposure was the time from hospital arrival to surgery (in hours). The primary outcome was mortality within 30 days. Secondary outcomes included a composite of mortality or other medical complications (MI, DVT, PE, and pneumonia) also within 30 days. Risk-adjusted cubic splines modeled the probability of each complication according to wait-time. The inflection point (in hours) when complications began to increase was used to define ‘early’ and ‘delayed’ surgery. To evaluate the robustness of this definition, outcomes amongst propensity-score matched early and delayed patients were compared using percent absolute risk differences (% ARDs, with 95% confidence intervals [CIs]). There were 42,230 patients who met entry criteria. Their mean age was 80.1 (±10.7) and the majority were female (70.5%). The risk of complications modeled by cubic splines consistently increased when wait-times were greater than 24 hours, irrespective of the complication considered. Compared to 13,731 propensity-score matched patients who received surgery earlier, 13,731 patients receiving surgery after 24 hours had a significantly higher risk of 30-day mortality (N=898 versus N=790, % ARD 0.79 [95% CI 0.23 to 1.35], p = .006) and the composite outcome (N=1,680 versus N=1,383, % ARD 2.16 [95% CI 1.43 to 2.89], p < .001). Overall, there were 14,174 patients (33.6%) who received surgery within 24 hours and 28,056 patients (66.4%) who received surgery after 24 hours. Increased wait-time was associated with a greater risk for 30-day mortality and other complications. The finding that a wait-time of 24 hours represents a threshold defining higher risk may inform existing hip fracture guidelines. Since two-thirds of patients did not receive surgery within this timeframe, performance improvement efforts that reduce wait-times are warranted.

We evaluated whether presenting information about the benefits of surgery in absolute and relative terms affects an individual’s decision to accept or reject alternative surgical procedures in hip fracture management. Our findings show how framing risk in relative terms affects the perception of risk and influences patient choice. Surgeons must use care in utilizing relative risk reduction in the absence of actual risk data. To evaluate whether presenting information about the benefits of surgery in absolute and relative terms affects an individual’s decision to accept or reject alternative surgical procedures in hip fracture management. We administered a face-to-face survey to fifty patients attending the fracture clinic. We asked patients to consider a scenario and to decide which treatment alternative they preferred based upon risk presentation. We presented risk in five ways: absolute risk difference, relative risk reduction, relative risk, number needed to treat, and odds ratio. Patients were most likely to favor internal fixation when the mortality results comparing internal versus arthroplasty were presented as a relative risk reduction. Patients continued to favor internal fixation despite being presented with a significantly increased risk of revision surgery. Lower level of education and those patients who had not experienced a fracture were significantly associated with their perceptions about method of presentation. Our findings show how framing risk in relative terms affects the perception of risk and influences patient choice. Patients concerns about mortality, even if non-significant differences are presented, outweigh concerns about significant increases in revision surgery with internal fixation. Surgeons must use care in utilizing relative risk reduction in the absence of actual risk data given our findings that may over-estimate the relative benefits of one procedure over another.
We evaluated growth and remodeling of the 21 hips after valgus osteotomy with rotational and sagittal components for hinge abduction in 21 patients (mean, 9.7 years) with Legg-Calvé-Perthes disease (LCPD) both clinically and radiologically. The type of osteotomy was determined by assessing the hinge movement three-dimensionally using preoperative gait assessment, 2-dimensional/3-dimensional computed tomography (2D/3D-CT), and intraoperative dynamic arthrography. The Iowa hip score averaged 66 (34 to 76) before operation and 92 (80 to 100) at a mean follow-up of 7.1 years (3.0 to 15.0). Radiographic measurements revealed favorable remodeling of the femoral head and improved hip joint mechanics. Valgus osteotomy with rotational and sagittal components results in sustained improvement in symptoms and functions and beneficially influences remodeling of the hip. Preoperative gait assessment, 2D/3D-CT, and intra-operative dynamic arthrography are helpful for assessing the spatial features of the femoral capital hump and for determining the optimal congruent position of the hip.
Aims. To identify factors influencing clinicians’ decisions to undertake a nonoperative hip fracture management approach among older people, and to determine whether there is global heterogeneity regarding these factors between clinicians from high-income countries (HIC) and low- and middle-income countries (LMIC). Methods. A SurveyMonkey questionnaire was electronically distributed to clinicians around the world through the Fragility Fracture Network (FFN)’s Perioperative Special Interest Group and clinicians’ personal networks between 24 May and 25 July 2021. Analyses were performed using Excel and STATA v16.0. Between-group differences were determined using independent-samples t-tests and chi-squared tests. Results. A total of 406 respondents from 51 countries answered the questionnaire, of whom 225 came from HIC and 180 from LMIC. Clinicians from HIC reported a greater median and mean estimated proportion of admitted patients with hip fracture undergoing surgery (median 96% (interquartile range (IQR) 95% to 99%); mean 94% (SD 8%)) than those from LMIC (median 85% (IQR 75% to 95%); mean 81% (SD 16%); p < 0.001). Global heterogeneity seems to exist regarding factors such as anticipated life expectancy, insufficient resources, ability to pay, treatment costs, and perception of risk in hip fracture management
Aims. Perthes’ disease is an idiopathic avascular necrosis of the developing femoral head, often causing deformity that impairs physical function. Current treatments aim to optimize the joint reaction force across the hip by enhancing congruency between the acetabulum and femoral head. Despite a century of research, there is no consensus regarding the optimal treatment. The aim of this study was to describe the experiences of children, their families, and clinicians when considering the treatment of Perthes’ disease. Methods. A qualitative study gathered information from children and their families affected by Perthes’ disease, along with treating clinicians. Interviews followed a coding framework, with the interview schedule informed by behavioural theory and patient and public involvement. Transcripts were analyzed using the framework method. Results. A total of 24 interviews took place, with 12 child/family dyads and 12 clinicians from UK NHS centres. Interviews identified widespread variation of routine care.
