Surgeons have commonly used modular femoral heads and stems from
different manufacturers, although this is not recommended by orthopaedic
companies due to the different manufacturing processes. We compared the rate of corrosion and rate of wear at the trunnion/head
taper junction in two groups of retrieved hips; those with mixed
manufacturers (MM) and those from the same manufacturer (SM). We identified 151 retrieved hips with large-diameter cobalt-chromium
heads; 51 of two designs that had been paired with stems from different
manufacturers (MM) and 100 of seven designs paired with stems from
the same manufacturer (SM). We determined the severity of corrosion
with the Goldberg corrosion score and the volume of material loss
at the head/stem junction. We used multivariable statistical analysis
to determine if there was a significant difference between the two

Until now, there has been no consensus as to whether stemmed
acetabular components are appropriate for use in patients undergoing
revision total hip arthroplasty (THA) who have major acetabular
defects or pelvic discontinuity. We wished to address this deficiency
in the literature. We carried out a retrospective study of 35 patients (six men
and 29 women) with a mean age of 68 years (37 to 87), with major
acetabular defects who underwent revision THA between 2000 and 2012.Aim

The goals of this study were to define the risk factors, characteristics,
and chronology of fractures in 5417 revision total hip arthroplasties
(THAs). From our hospital’s prospectively collected database we identified
all patients who had undergone a revision THA between 1969 and 2011
which involved the femoral stem. The patients’ medical records and
radiographs were examined and the relevant data extracted. Post-operative
periprosthetic fractures were classified using the Vancouver system.
A total of 5417 revision THAs were identified.Aims
Patients and Methods
The popularity of cementless total hip replacement
(THR) has surpassed cemented THR in England and Wales. This retrospective
cohort study records survival time to revision following primary
cementless THR with the most common combination (accounting for
almost a third of all cementless THRs), and explores risk factors independently
associated with failure, using data from the National Joint Registry
for England and Wales. Patients with osteoarthritis who had a DePuy
Corail/Pinnacle THR implanted between the establishment of the registry
in 2003 and 31 December 2010 were included within analyses. There
were 35 386 procedures. Cox proportional hazard models were used
to analyse the extent to which the risk of revision was related
to patient, surgeon and implant covariates. The overall rate of
revision at five years was 2.4% (99% confidence interval 2.02 to
2.79). In the final adjusted model, we found that the risk of revision
was significantly higher in patients receiving metal-on-metal (MoM:
hazard ratio (HR) 1.93, p <
0.001) and ceramic-on-ceramic bearings
(CoC: HR 1.55, p = 0.003) compared with the best performing bearing
(metal-on-polyethylene). The risk of revision was also greater for
smaller femoral stems (sizes 8 to 10: HR 1.82, p <
0.001) compared
with mid-range sizes. In a secondary analysis of only patients where body
mass index (BMI) data were available (n = 17 166), BMI ≥ 30 kg/m2 significantly
increased the risk of revision (HR 1.55, p = 0.002). The influence
of the bearing on the risk of revision remained significant (MoM:
HR 2.19, p <
0.001; CoC: HR 2.09,
p = 0.001). The risk of revision was independent of age, gender,
head size and offset, shell, liner and stem type, and surgeon characteristics. We found significant differences in failure between bearing surfaces
and femoral stem size after adjustment for a range of covariates
in a large cohort of single-brand cementless THRs. In this study
of procedures performed since 2003, hard bearings had significantly
higher rates of revision, but we found no evidence that head size
had an effect. Patient characteristics, such as BMI and American
Society of Anesthesiologists grade, also influence the survival

The aim of this randomised controlled study was
to compare functional and radiological outcomes between modern cemented
and uncemented hydroxyapatite coated stems after one year in patients
treated surgically for a fracture of the femoral neck. A total of
141 patients aged >
65 years were included. Patients were randomised
to be treated with a cemented Exeter stem or an uncemented Bimetric
stem. The patients were reviewed at four and 12 months. The cemented group performed better than the uncemented group
for the Harris hip score (78 In conclusion, our data do not support the use of an uncemented
hydroxyapatite coated stem for the treatment of displaced fractures

Periprosthetic femoral fracture (PFF) is a potentially
devastating complication after total hip arthroplasty, with historically
high rates of complication and failure because of the technical
challenges of surgery, as well as the prevalence of advanced age
and comorbidity in the patients at risk. This study describes the short-term outcome after revision arthroplasty
using a modular, titanium, tapered, conical stem for PFF in a series
of 38 fractures in 37 patients. The mean age of the cohort was 77 years (47 to 96). A total of
27 patients had an American Society of Anesthesiologists grade of
at least 3. At a mean follow-up of 35 months (4 to 66) the mean
Oxford Hip Score (OHS) was 35 (15 to 48) and comorbidity was significantly
associated with a poorer OHS. All fractures united and no stem needed
to be revised. Three hips in three patients required further surgery
for infection, recurrent PFF and recurrent dislocation and three
other patients required closed manipulation for a single dislocation.
One stem subsided more than 5 mm but then stabilised and required
no further intervention. In this series, a modular, tapered, conical stem provided a versatile

High failure rates of metal-on-metal hip arthroplasty implants have highlighted the need for more careful introduction and monitoring of new implants and for the evaluation of the safety of medical devices. The National Joint Registry and other regulatory services are unable to detect failing implants at an early enough stage. We aimed to identify validated surrogate markers of long-term outcome in patients undergoing primary total hip arthroplasty (THA). We conducted a systematic review of studies evaluating surrogate markers for predicting long-term outcome in primary THA. Long-term outcome was defined as revision rate of an implant at ten years according to National Institute of Health and Care Excellence guidelines. We conducted a search of Medline and Embase (OVID) databases. Separate search strategies were devised for the Cochrane database and Google Scholar. Each search was performed to include articles from the date of their inception to June 8, 2015.Objectives
Methods
Many different lengths of stem are available
for use in primary total hip replacement, and the morphology of
the proximal femur varies greatly. The more recently developed shortened
stems provide a distribution of stress which closely mimics that
of the native femur. Shortening the femoral component potentially
comes at the cost of decreased initial stability. Clinical studies
on the performance of shortened cemented and cementless stems are promising,
although long-term follow-up studies are lacking. We provide an
overview of the current literature on the anatomical features of
the proximal femur and the biomechanical aspects and clinical outcomes
associated with the length of the femoral component in primary hip
replacement, and suggest a classification system for the length

This was a randomised controlled trial studying
the safety of a new short metaphyseal fixation (SMF) stem. We hypothesised
that it would have similar early clinical results and micromovement
to those of a standard-length tapered Synergy metaphyseal fixation
stem. Using radiostereometric analysis (RSA) we compared the two
stems in 43 patients. A short metaphyseal fixation stem was used
in 22 patients and a Synergy stem in 21 patients. No difference
was found in the clinical outcomes pre- or post-operatively between
groups. RSA showed no significant differences two years post-operatively
in mean micromovement between the two stems (except for varus/valgus tilt
at p = 0.05) (subsidence 0.94 mm (. sd. 1.71) vs 0.32
mm (. sd. 0.45), p = 0.66; rotation 0.96° (. sd. 1.49) vs 1.41°
(. sd. 2.95), p = 0.88; and total migration 1.09 mm (. sd. 1.74) vs 0.73
mm (. sd. 0.72), p = 0.51). A few stems (four SMF and three Synergy)
had initial migration >
1.0 mm but stabilised by three to six months,
with the exception of one SMF stem which required revision three
years post-operatively. For most stems, total micromovement was
very low at two years (subsidence <
0.5 mm, rotation <
1.0°,
total migration <
0.5 mm), which was consistent with osseous ingrowth.

Revision total hip arthroplasty (THA) is challenging
when there is severe loss of bone in the proximal femur. The purpose
of this study was to evaluate the clinical and radiographic outcomes
of revision THA in patients with severe proximal femoral bone loss
treated with a fluted, tapered, modular femoral component. Between
January 1998 and December 2004, 92 revision THAs were performed
in 92 patients using a single fluted, tapered, modular femoral stem
design. Pre-operative diagnoses included aseptic loosening, infection
and peri-prosthetic fracture. Bone loss was categorised pre-operatively
as Paprosky types III-IV, or Vancouver B3 in patients with a peri-prosthetic
fracture. The mean clinical follow-up was 6.4 years (2 to 12). A
total of 47 patients had peri-operative complications, 27 of whom
required further surgery. However, most of these further operations
involved retention of a well-fixed femoral stem, and 88/92 femoral
components (97%) remained Revision THA in patients with extensive proximal femoral bone
loss using the Link MP fluted, tapered, modular stem led to a high

Recent guidance recommends the use of a well-proven
cemented femoral stem for hemiarthroplasty in the management of
fractures of the femoral neck, and the Exeter Trauma Stem (ETS)
has been suggested as an example of such an implant. The design
of this stem was based on the well-proven Exeter Total Hip Replacement
stem (ETHRS). This study assessed the surface finish of the ETS
in comparison with the ETHRS. Two ETSs and two ETHRSs were examined
using a profilometer with a precision of 1 nm and compared with
an explanted Exeter Matt stem. The mean roughness average (RA) of

Noise generation has been reported with ceramic-on-ceramic
articulations in total hip replacement (THR). This study evaluated
208 consecutive Delta Motion THRs at a mean follow-up of 21 months
(12 to 35). There were 141 women and 67 men with a mean age of 59
years (22 to 84). Patients were reviewed clinically and radiologically,
and the incidence of noise was determined using a newly described
assessment method. Noise production was examined against range of
movement, ligamentous laxity, patient-reported outcome scores, activity
level and orientation of the acetabular component. There were 143
silent hips (69%), 22 (11%) with noises other than squeaking, 17
(8%) with unreproducible squeaking and 26 (13%) with reproducible
squeaking. Hips with reproducible squeaking had a greater mean range
of movement (p <
0.001) and mean ligament laxity (p = 0.004), smaller
median head size (p = 0.01) and decreased mean acetabular component
inclination (p = 0.02) and anteversion angle (p = 0.02) compared
with the other groups. There was no relationship between squeaking
and age (p = 0.13), height (p = 0.263), weight (p = 0.333), body
mass index (p = 0.643), gender (p = 0.07) or patient outcome score
(p = 0.422). There were no revisions during follow-up. Despite the
surprisingly high incidence of squeaking, all patients remain satisfied

This was a safety study where the hypothesis was that the newer-design CPCS femoral stem would demonstrate similar early clinical results and micromovement to the well-established Exeter stem. Both are collarless, tapered, polished cemented stems, the only difference being a slight lateral to medial taper with the CPCS stem. A total of 34 patients were enrolled in a single-blinded randomised controlled trial in which 17 patients received a dedicated radiostereometric CPCS stem and 17 a radiostereometric Exeter stem. No difference was found in any of the outcome measures pre-operatively or post-operatively between groups. At two years, the mean subsidence for the CPCS stem was nearly half that seen for the Exeter stem (0.77 mm (−0.943 to 1.77) and 1.25 mm (0.719 to 1.625), respectively; p = 0.032). In contrast, the mean internal rotation of the CPCS stem was approximately twice that of the Exeter (1.61° (−1.07° to 4.33°) and 0.59° (0.97° to 1.64°), respectively; p = 0.048). Other migration patterns were not significantly different between the stems. The subtle differences in designs may explain the different patterns of migration. Comparable migration with the Exeter stem suggests that the CPCS design will perform well in the long term.
In 2005, we demonstrated that the polished triple-tapered
C-stem at two years had migrated distally and rotated internally.
From that series, 33 patients have now been followed radiologically,
clinically and by radiostereometric analysis (RSA) for up to ten
years. The distal migration within the cement mantle had continued
and reached a mean of 2 mm (0.5 to 4.0) at ten years. Internal rotation,
also within the cement mantle, was a mean 3.8° (external 1.6° to
internal 6.6°) The cement mantle did not show any sign of migration
or loosening in relation to the femoral bone. There were no clinical
or radiological signs indicating that the migration or rotation
within the cement mantle had had any adverse effects for the patients. Cite this article:
Squeaking arising from a ceramic-on-ceramic (CoC)
total hip replacement (THR) may cause patient concern and in some
cases causes patients to seek revision surgery. We performed a meta-analysis
to determine the incidence of squeaking and the incidence of revision
surgery for squeaking. A total of 43 studies including 16 828 CoC
THR that reported squeaking, or revision for squeaking, were entered
into the analysis. The incidence of squeaking was 4.2% and the incidence
of revision for squeaking was 0.2%. The incidence of squeaking in
patients receiving the Accolade femoral stem was 8.3%, and the incidence
of revision for squeaking in these patients was 1.3%. Cite this article:
Conventional uncemented femoral implants provide
dependable long-term fixation in patients with a wide range of functional
requirements. Yet challenges associated with proximal–distal femoral
dimensional mismatch, preservation of bone stock, and minimally
invasive approaches have led to exploration into alternative implant designs.
Short stem designs focusing on a stable metaphyseal fit have emerged
to address these issues in total hip replacement (THR). Uncemented
metaphyseal-engaging short stem implants are stable and are associated
with proximal bone remodeling closer to the metaphysis when compared
with conventional stems and they also have comparable clinical performances.
Short stem metaphyseal-engaging implants can meet the goals of a
successful THR, including tolerating a high level of patient function,

The National Institute for Health and Clinical
Excellence (NICE) guidelines from 2011 recommend the use of cemented
hemi-arthroplasty for appropriate patients with an intracapsular
hip fracture. In our institution all patients who were admitted
with an intracapsular hip fracture and were suitable for a hemi-arthroplasty
between April 2010 and July 2012 received an uncemented prosthesis
according to our established departmental routine practice. A retrospective
analysis of outcome was performed to establish whether the continued
use of an uncemented stem was justified. Patient, surgical and outcome
data were collected on the National Hip Fracture database. A total
of 306 patients received a Cathcart modular head on a Corail uncemented

We report a multicentre prospective consecutive
study assessing the long-term outcome of the proximally hydroxyapatite
(HA)-coated ABG II monobloc femoral component in a series of 1148
hips in 1053 patients with a mean age at surgery of 64.77 years
(22 to 80) at a mean follow-up of 10.84 years (10 to 15.25). At
