1. A high incidence of paraplegia following operations for the correction of severe scoliosis in adults led to an investigation of the normal blood supply of the human spinal cord. 2. This entailed three methods of study: micro-dissection of the vessels of the spinal cord in thirty-five cadavers; radiological measurements of the spinal canal in fifty healthy subjects; and a study of the macerated spinal column in six adult cadavers. 3. The blood supply of the spinal cord is shown to be least rich, and the spinal canal narrowest, from the T.4 to approximately the T.9 vertebral level. This is named the critical vascular zone of the spinal cord, the zone in which interference with the circulation is most likely to result in paraplegia

1. Ten children with scoliosis and cerebral palsy of various types have been reviewed. All underwent operation for correction and stabilisation of the spinal curve, and spinal fusion. 2. Three of the ten patients required supplemental surgery in the form of regrafting, with or without reinsertion of Harrington rods. All have shown considerable correction of the curve, and in all cases the fusion appears to be consolidated. Operation has given these children stable spines which are compensated. Their ability to sit, and in some cases to stand, has improved. 3. In many such cases of scoliosis complicating cerebral palsy Harrington instrumentation and spinal fusion is the only feasible effective form of treatment

Early decompression in Pott's paraplegia gives encouraging results. It produces speedy recovery from paraplegia and ensures rapid healing of the lesion. Lateral extrapleural decompression without fusion for lesions of thoracic vertebrae is safe and satisfactory. It gives adequate exposure of the anterior and posterior parts of the vertebral bodies and of the theca, without endangering the stability of the spine. Age, sex and site of the lesion have no influence on the prognosis, whereas paraplegia of longer duration, paraplegia in flexion, and paraplegia presenting as a spinal cord tumour carry a bad prognosis. In early lesions there is reconstitution of vertebral bodies whereas in advanced lesions there is consolidation or bony fusion

1. A twenty-five-year-old man who developed neuropathic arthritis because of indifference to pain is described. He presented at the age of thirteen with degenerative changes in the light knee with analgesia only in the right leg below the knee. 2. A painless fracture of the ulna developed non-union and was associated with a neuropathic arthritis of the elbow joint. 3. Later the other knee and the lumbo-sacral spine became affected. The analgesia became generalised with all other senses staying intact. Apart from tonic pupillary reactions and diminished corneal sensibility no other neurological signs could be found. 4. A deceased sister may also have suffered from the same condition

In 21 children with myelomeningocele who underwent kyphectomy for congenital kyphosis of the lumbar spine, aortography revealed no case in which the aorta followed the spinal curvature. Many anomalies of the intercostal and segmental arteries were demonstrated which were only in part associated with deformities of the respective vertebral bodies. The kidneys, which were frequently malformed, often lay within the kyphosis and were therefore at risk of operative damage. We conclude that the aorta is not at risk and that aortography is not usually necessary before kyphectomy, except in patients who have undergone prior abdominal surgery. Non-invasive methods (ultrasound, CT or MRI) should be used to detect malpositions and malformations of the kidneys

We have reviewed 41 patients with pustulotic arthro-osteopathy (PAO), all having both the typical skin rash of pustulosis palmaris et plantaris and bone lesions. The most common bones affected were the clavicle, sternum and ribs. Changes in the clavicle started, not as an enthesopathy, but with periosteal bone formation, indicative of a bone marrow disorder. About 30% of the patients also had lesions in the spine, sacroiliac region or the peripheral joints. Bone and joint lesions followed a variable and intermittent clinical course over a long period of time. Biopsies in eight cases showed similar inflammatory changes in skin, bone and synovium, with infiltration of lymphocytes and polymorphonuclear leucocytes. This suggests that there is a common pathogenesis in the three tissues

The midsagittal and interpedicular diameters and the trefoil shape of lumbar vertebrae of known age at death were measured in skeletons from a population aged between 1 and 70 years. All the trefoil configurations were at L5 with the exception of one at L4. The overall prevalence was 25%, but this shape was not generally apparent until adulthood. The midsagittal diameter in the trefoil canals was found to be significantly smaller than that in the unaffected canals. This did not change significantly after six years of age indicating that the cause of the trefoil configuration is probably present early in life. The trefoil shape was no more common in the spines of the elderly subjects. Our findings indicate that the trefoil configuration of the lumbar vertebral canal has a developmental origin and is not a consequence of degenerative processes

Seven patients treated in infancy by a lumbar theco-peritoneal shunt for idiopathic communicating hydrocephalus presented later in childhood after developing a characteristic orthopaedic syndrome. This included a severe, rigid and progressive lumbar hyperlordosis, severe bilateral restriction of straight leg raising and abnormalities of stance and gait. Four of the patients, who had severe hyperlordotic curves of over 90 degrees, required operations to correct their extreme deformity. The recommended method of correction is a three-stage procedure: first, anterior wedge resection osteotomies at several levels in the lumbar spine, then a period of "90-90" femoral traction, and finally a posterior fusion and stabilisation using Harrington instrumentation. The results were good, with few complications

We studied the possible role of melatonin deficiency in experimentally-induced scoliosis. A total of 90 chickens underwent pinealectomy on the third day after hatching: 30 were treated with serotonin, 30 with melatonin and 30 received no therapy (control group). Scoliosis developed in all the control group, in 22 of the serotonin group, and in only 6 of the melatonin group. The six melatonin-treated chickens with scoliosis had less severe spinal deformities than those in the serotonin-treated group. There were lower blood melatonin concentrations in chickens with scoliosis than in those without. Our findings suggest that melatonin deficiency contributes to the aetiology of this experimental scoliosis, probably by interfering with the normally symmetrical growth of the proprioceptive system involving the paraspinal muscles and the spine

Thirty-one patients who had been treated surgically for lumbar spinal stenosis between 1968 and 1978 at the Norfolk and Norwich Hospital have been personally reviewed. In 28 patients (90 per cent) degenerative change in the lumbar spine had been the principal aetiological factor; the other three had idiopathic developmental lumbar spinal stenosis. In 17 patients (55 per cent) the result was classified as good, although a total of 26 patients (84 per cent) were satisfied. In patients with degenerative changes, one important reason for failure was inadequate decompression; re-operation in this group seemed worthwhile. The good results of operation for lumbar spinal stenosis were characterised by rapid resolution of pain in the leg

1. A thirty-two-year-old man has been under treatment for twelve years for primary hydatid disease of the body of the third lumbar vertebra. 2. The second lumbar vertebra became involved, as were the soft tissues over a wide area, but the spinal canal was not affected. 3. A hydatid cyst was found in the left lung. 4. Management included stabilisation of the spine by bone graft, desensitisation to hydatid antigen, removal of the diseased vertebral bodies, pedicles and processes, the use of saturated solution of sodium chloride as a scolicide and the dilation of secondary ureteral strictures. 5. The patient, although not cured, has been enabled to "live with his disease" to earn his living and enjoy tennis and swimming

We reviewed 29 patients who had developed destructive arthropathy of the spine during long-term haemodialysis. Their mean age when haemodialysis began was 43.8 years; at diagnosis they had been dialysed for an average of 8.6 years. In 26 patients, the lesions were between C4 and C7; in six they were between L4 and S1, three having lesions in both regions. Sixteen patients had had previous surgery for carpal tunnel syndrome. Spinal surgery was performed in nine patients with satisfactory results in only five. We demonstrated beta-2 microglobulin amyloid deposits in the discs and surrounding ligaments in all biopsied cases. The natural history and management of this condition are not yet clear

Recent studies of the fibroblast growth factor receptor 3 (FGFR3) gene have established that achondroplasia and hypochondroplasia are allelic disorders of different mutations. To determine whether the genotype could be distinguished on the basis of the phenotype, we analysed height, arm span, and skeletal radiographs from 23 patients with achondroplasia and the G380R mutation of FGFR3 and eight with hypochondroplasia and the N540K mutation. Both conditions share the classical pathological features of micromelic short stature, reduced or unchanged interpedicular distances in the lumbar spine, disproportionately long fibulae, and squared and shortened pelvic ilia. These were significantly more severe in the G380R patients than in the N540K patients. Our findings have shown a firm statistical correlation between the genotype and the phenotype, although there were a few exceptional cases in which there was phenotypic overlap between the two conditions

We have measured the dynamic movement of the lumbar spine in 57 patients with degenerative lumbar disc disease. Each completed a questionnaire which recorded pain and subjective signs and symptoms. From plain lateral radiographs, the subjects were graded using the criteria of Kellgren and Lawrence and those of Lane et al, which are both based on the severity of degenerative changes. Measurements of the height of the disc space and the vertebral height were obtained and expressed as a ratio. We found no relationship between the characteristics of spinal movement and the overall grading of degenerative disc disease with either system. Both were influenced (p < 0.01) by age, walking distance, severity of symptoms, drug intake and frequency of pain. The present systems for grading degenerative disc disease from plain lateral radiographs have limited application

There is a need for the accurate measurement of the outcome after knee surgery. The Oxford Knee Score is being increasingly used since it is reported to be short, simple, inexpensive and validated. We sent the questionnaire to 346 patients awaiting surgery to the hip or lumbar spine. Only 11% of 141 patients with proximal pathology who denied knee problems gave a maximum score. Their mean score was substantially lower than expected at 28.7 (maximum 48), and was significantly lower than the score of 36.5 obtained from patients after total knee replacement. We therefore suggest that the frequent coexistence of hip or spinal pathology will significantly alter both the absolute score and any improvement to be expected after knee surgery. Although sensitive to disability originating from the knee the Oxford Knee Score is not sufficiently specific since it is heavily influenced by more proximal pathology

A new technique of shoulder fusion is presented using a posterior approach. After removal of the articular cartilage, a Rush pin is introduced from the spine of the scapula, through the glenoid into the medullary canal of the humerus. This is supplemented by tension-band wiring from the acromion to the neck of the humerus and a muscle pedicle graft attached to the acromion. A shoulder spica is applied for four to six weeks. Four patients with injuries to the upper brachial plexus and 14 with paralysis of the upper arm due to anterior poliomyelitis have been followed for three years. One of the 18 patients developed nonunion; she had removed her own cast prematurely. This method of fixation provides high shear resistance and low axial stiffness without deforming plastically. It does not affect bone growth in young patients, is effective in patients with osteoporosis, and gives a high rate of union

Late-onset idiopathic scoliosis is associated with a rib hump in the thoracic region, and surgery is indicated when this deformity becomes unacceptable. Fifty patients with this deformity were treated by the Leeds procedure, which consists of segmental wiring to a kyphotically-contoured square-ended Harrington rod; this procedure not only derotates the spine but restores the natural thoracic kyphosis, thus avoiding subsequent buckling. All patients were followed up for a minimum of two years. Forty-two of these, who had a pre-operative Cobb angle of less than 60 degrees, were treated by one-stage instrumentation and fusion, while the remaining eight with greater curves underwent preliminary anterior multiple discectomy to provide flexibility with shortening. Postoperative loss of correction was not observed and there were no neurological complications

Cadaveric lumbar discs were injected with chymopapain and subjected to a series of mechanical tests over a period of up to 19 hours. Discs from the same spine injected with saline were used as controls. The results showed that chymopapain had no measurable effect on the mechanical properties of the disc apart from the increased height and stiffening caused by fluid injection. Another series of tests on isolated pieces of disc material showed that chymopapain could reduce the size of prolapsed nuclear material by 24% in one hour and by 80% in 48 hours. It is concluded that, in the short-term, chymopapain has a negligible effect on the mechanics of a disc but it can reduce the size of any prolapsed nuclear material with which it comes in contact

Eleven patients with metastases in the cervical spine had operations for severe pain due to a pathological fracture which, in eight of the cases, was unstable. Conservative treatment had either failed or was unsuitable. In the first five patients internal fixation with tension band wires and bone grafts was supported by a halo-brace. The method has evolved to the use of posterior instrumentation with laminar wires from two levels above the lesion to two levels below. Stability is increased by the use of bone cement with or without bone autografts applied to one side of the spine. This technique enables the patient to get up within a few days of operation--a great advantage when life expectancy is limited. The operations were successful in all except one case
