The interaction between the lumbosacral spine
and the pelvis is dynamically related to positional change, and
may be complicated by co-existing pathology. This review summarises
the current literature examining the effect of sagittal spinal deformity
on pelvic and acetabular orientation during total hip arthroplasty
(THA) and provides recommendations to aid in placement of the acetabular
component for patients with co-existing spinal pathology or long
spinal fusions. Pre-operatively, patients can be divided into four
categories based on the flexibility and sagittal balance of the
spine. Using this information as a guide, placement of the acetabular
component can be optimal based on the type and significance of co-existing

Dysplasia of the hip, hypotonia, osteopenia,
ligamentous laxity, and mental retardation increase the complexity
of performing and managing patients with Down syndrome who require
total hip replacement (THR). We identified 14 patients (six males,
eight females, 21 hips) with Down syndrome and degenerative disease
of the hip who underwent THR, with a minimum follow-up of two years

Squeaking arising from a ceramic-on-ceramic (CoC)
total hip replacement (THR) may cause patient concern and in some
cases causes patients to seek revision surgery. We performed a meta-analysis
to determine the incidence of squeaking and the incidence of revision
surgery for squeaking. A total of 43 studies including 16 828 CoC
THR that reported squeaking, or revision for squeaking, were entered
into the analysis. The incidence of squeaking was 4.2% and the incidence
of revision for squeaking was 0.2%. The incidence of squeaking in
patients receiving the Accolade femoral stem was 8.3%, and the incidence

Orientation of the acetabular component influences
wear, range of movement and the incidence of dislocation after total
hip replacement (THR). During surgery, such orientation is often
referenced to the anterior pelvic plane (APP), but APP inclination
relative to the coronal plane (pelvic tilt) varies substantially
between individuals. In contrast, the change in pelvic tilt from
supine to standing (dPT) is small for nearly all individuals. Therefore,
in THR performed with the patient supine and the patient’s coronal
plane parallel to the operating table, we propose that freehand placement
of the acetabular component placement is reliable and reflects standing
(functional) cup position. We examined this hypothesis in 56 hips
in 56 patients (19 men) with a mean age of 61 years (29 to 80) using
three-dimensional CT pelvic reconstructions and standing lateral
pelvic radiographs. We found a low variability of acetabular component
placement, with 46 implants (82%) placed within a combined range
of 30° to 50° inclination and 5° to 25° anteversion. Changing from
the supine to the standing position (analysed in 47 patients) was associated
with an anteversion change <

Accurate placement of the acetabular component during total hip
arthroplasty (THA) is an important factor in the success of the
procedure. However, the reported accuracy varies greatly and is
dependent upon whether free hand or navigated techniques are used.
The aim of this study was to assess the accuracy of an instrument
system that incorporates 3D printed, patient-specific guides designed
to optimise the placement of the acetabular component. A total of 100 consecutive patients were prospectively enrolled
and the accuracy of placement of the acetabular component was measured
using post-operative CT scans.Aims
Patients and Methods
The purpose of this study was to compare the
amount of acetabular bone removed during hip resurfacing (HR) and cementless
total hip replacement (THR), after controlling for the diameter
of the patient’s native femoral head. Based on a power analysis,
64 consecutive patients (68 hips) undergoing HR or THR were prospectively
enrolled in the study. The following data were recorded intra-operatively:
the diameter of the native femoral head, the largest reamer used,
the final size of the acetabular component, the size of the prosthetic
femoral head and whether a decision was made to increase the size
of the acetabular component in order to accommodate a larger prosthetic femoral

Between 1999 and 2001, 90 patients underwent
total hip replacement using the same uncemented acetabular and femoral
components with a 28 mm metallic femoral head but with prospective
randomisation of the acetabular liner to either Durasul highly cross-linked
polyethylene or nitrogen-sterilised Sulene polyethylene. We assessed
83 patients at a minimum follow-up of ten years. Linear penetration
of the femoral head was estimated at six weeks, six and 12 months
and annually thereafter, using the Dorr method, given the non-spherical
shape of the acetabular component. There was no loosening of any component; only one hip in the
Sulene group showed proximal femoral osteolysis. The mean penetration
of the femoral head at six weeks was 0.08 mm (0.02 to 0.15) for
the Durasul group and 0.16 mm (0.05 to 0.28) for the Sulene group

We describe the clinical and radiological results
of cementless primary total hip replacement (THR) in 25 patients
(18 women and seven men; 30 THRs) with severe developmental dysplasia
of the hip (DDH). Their mean age at surgery was 47 years (23 to
89). In all, 21 hips had Crowe type III dysplasia and nine had Crowe
type IV. Cementless acetabular components with standard polyethylene
liners were introduced as close to the level of the true acetabulum
as possible. The modular cementless S-ROM femoral component was
used with a low resection of the femoral neck. A total of 21 patients (25 THRs) were available for review at
a mean follow-up of 18.7 years (15.8 to 21.8). The mean modified
Harris hip score improved from 46 points pre-operatively to 90 at
final follow up (p <
0.001). A total of 15 patients (17 THRs; 57%) underwent revision of the
acetabular component at a mean of 14.6 years (7 to 20.8), all for
osteolysis. Two patients (two THRs) had symptomatic loosening. No
patient underwent femoral revision. Survival with revision of either
component for any indication was 81% at 15 years (95% CI 60.1 to
92.3), with 21 patients at risk. This technique may reduce the need for femoral osteotomy in severe

In order to prevent dislocation of the hip after total hip arthroplasty
(THA), patients have to adhere to precautions in the early post-operative
period. The hypothesis of this study was that a protocol with minimal
precautions after primary THA using the posterolateral approach
would not increase the short-term (less than three months) risk
of dislocation. We prospectively monitored a group of unselected patients undergoing
primary THA managed with standard precautions (n = 109, median age
68.9 years; interquartile range (IQR) 61.2 to 77.3) and a group
who were managed with fewer precautions (n = 108, median age 67.2
years; IQR 59.8 to 73.2). There were no significant differences between
the groups in relation to predisposing risk factors. The diameter
of the femoral head ranged from 28 mm to 36 mm; meticulous soft-tissue
repair was undertaken in all patients. The medical records were
reviewed and all patients were contacted three months post-operatively
to confirm whether they had experienced a dislocation. Aims

Ceramic-on-ceramic bearings in hip replacement have low rates of wear and are increasingly being used in young adults. Our aim was to determine the incidence of audible phenomena or other bearing-related complications. We retrospectively analysed 250 ceramic-on-ceramic hip replacements in 224 patients which had been implanted between April 2000 and December 2007. The mean age of the patients at operation was 44 years (14 to 83) and all the operations were performed using the same surgical technique at a single centre. At a mean follow-up of 59 months (24 to 94), the mean Oxford hip score was 40.89 (11 to 48). There were six revisions, three of which were for impingement-related complications. No patient reported squeaking, but six described grinding or clicking, which was usually associated with deep flexion. No radiological evidence of osteolysis or migration of the components was observed in any hip. The early to mid-term results of contemporary ceramic-on-ceramic hip replacement show promising results with few concerns in terms of noise and squeaking. Positioning of the acetabular component remains critical in regard to the reduction of other impingement-related complications.

Eighteen hip fusions were converted to total
hip replacements. A constrained acetabular liner was used in three hips.
Mean follow up was five years (two to 15). Two (11%) hips failed,
requiring revision surgery and two patients (11%) had injury to
the peroneal nerve. Heterotopic ossification developed in seven
(39%) hips, in one case resulting in joint ankylosis. No hips dislocated. Conversion of hip fusion to hip replacement carries an increased
risk of heterotopic ossification and neurological injury. We advise
prophylaxis against heterotropic ossification. When there is concern
about hip stability we suggest that the use of a constrained acetabular
liner is considered. Despite the potential for complications, this procedure
had a high success rate and was effective in restoring hip function.
We examined the one-year risk of symptomatic
venous thromboembolism (VTE) following primary total hip replacement
(THR) among Danish patients and a comparison cohort from the general
population. From the Danish Hip Arthroplasty Registry we identified
all primary THRs performed in Denmark between 1995 and 2010 (n =
85 965). In all, 97% of patients undergoing THR received low-molecular-weight
heparin products during hospitalisation. Through the Danish Civil
Registration System we sampled a comparison cohort who had not undergone
THR from the general population (n = 257 895). Among the patients
undergoing THR, the risk of symptomatic VTE was 0.79% between 0
and 90 days after surgery and 0.29% between 91 and 365 days after
surgery. In the comparison cohort the corresponding risks were 0.05%
and 0.12%, respectively. The adjusted relative risks of symptomatic
VTE among patients undergoing THR were 15.84 (95% confidence interval
(CI) 13.12 to 19.12) during the first 90 days after surgery and
2.41 (95% CI 2.04 to 2.85) during 91 to 365 days after surgery,
compared with the comparison cohort. The relative risk of VTE was
elevated irrespective of the gender, age and level of comorbidity
at the time of THR. We concluded that THR was associated with an increased risk of
symptomatic VTE up to one year after surgery compared with the general
population, although the absolute risk is small.
A high radiographic inclination angle (RI) contributes
to accelerated wear and has been associated with dislocation after
total hip arthroplasty (THA). With freehand positioning of the acetabular
component there is a lack of accuracy, with a trend towards a high
radiographic inclination angle. The aim of this study was to investigate
whether the use of a digital protractor to measure the operative
inclination angle (OI) could improve the positioning of the acetabular
component in relation to a ‘safe zone’. We measured the radiographic inclination angles of 200 consecutive
uncemented primary THAs. In the first 100 the component was introduced
freehand and in the second 100 a digital protractor was used to
measure the operative inclination angle. The mean difference between the operative and the radiographic
inclination angles
(∆RI–OI) in the second cohort was 12.3° (3.8° to 19.8°). There was
a strong correlation between the circumference of the hip and ∆RI–OI.
The number of RI outliers was significantly reduced in the protractor
group (p = 0.002). Adjusting the OI, using a digital protractor and taking into
account the circumference of the patient’s hip, improves the RI
