Minimally invasive total knee replacement (MIS-TKR)
has been reported to have better early recovery than conventional
TKR. Quadriceps-sparing (QS) TKR is the least invasive MIS procedure,
but it is technically demanding with higher reported rates of complications
and outliers. This study was designed to compare the early clinical
and radiological outcomes of TKR performed by an experienced surgeon
using the QS approach with or without navigational assistance (NA),
or using a mini-medial parapatellar (MP) approach. In all, 100 patients
completed a minimum two-year follow-up: 30 in the NA-QS group, 35
in the QS group, and 35 in the MP group. There were no significant
differences in clinical outcome in terms of ability to perform a
straight-leg raise at 24 hours (p = 0.700), knee score (p = 0.952),
functional score (p = 0.229) and range of movement (p = 0.732) among
the groups. The number of outliers for all three radiological parameters
of mechanical axis, frontal femoral component alignment and frontal
tibial component alignment was significantly lower in the NA-QS
group than in the QS group (p = 0.008), but no outlier was found
in the MP group. In conclusion, even after the surgeon completed a substantial
number of cases before the commencement of this study, the supplementary
intra-operative use of computer-assisted navigation with QS-TKR
still gave inferior radiological results and longer operating time,
with a similar outcome at two years when compared with a MP approach. Cite this article:
As many as 25% to 40% of unicompartmental knee
replacement (UKR) revisions are performed for pain, a possible cause
of which is proximal tibial strain. The aim of this study was to
examine the effect of UKR implant design and material on cortical
and cancellous proximal tibial strain in a synthetic bone model.
Composite Sawbone tibiae were implanted with cemented UKR components
of different designs, either all-polyethylene or metal-backed. The tibiae
were subsequently loaded in 500 N increments to 2500 N, unloading
between increments. Cortical surface strain was measured using a
digital image correlation technique. Cancellous damage was measured
using acoustic emission, an engineering technique that detects sonic
waves (‘hits’) produced when damage occurs in material. Anteromedial cortical surface strain showed significant differences
between implants at 1500 N and 2500 N in the proximal 10 mm only
(p <
0.001), with relative strain shielding in metal-backed implants.
Acoustic emission showed significant differences in cancellous bone
damage between implants at all loads (p = 0.001). All-polyethylene implants
displayed 16.6 times the total number of cumulative acoustic emission
hits as controls. All-polyethylene implants also displayed more
hits than controls at all loads (p <
0.001), more than metal-backed
implants at loads ≥ 1500 N (p <
0.001), and greater acoustic
emission activity on unloading than controls (p = 0.01), reflecting
a lack of implant stiffness. All-polyethylene implants were associated
with a significant increase in damage at the microscopic level compared
with metal-backed implants, even at low loads. All-polyethylene
implants should be used with caution in patients who are likely

The Oxford unicompartmental knee replacement gives good results in patients with symptomatic osteoarthritis of the medial compartment. Previous studies have suggested that the presence of radiolucent lines (RLLs) does not reflect a poor outcome in such patients. However, the reliability and validity of this assessment have not been determined. Our aim was to assess the intra- and interobserver reliability and the sensitivity and specificity of the assessment of RLLs around both tibial and femoral components using standard radiographs. Two reviewers assessed the radiographs of 45 patients who had loosening of the tibial or femoral component confirmed at revision surgery and compared them with those of a series of 45 asymptomatic patients matched for age and gender. The results suggested that, using standard radiographs, tibial RLLs were 63.6% sensitive and 94.4% specific and femoral RLLs 63.9% sensitive and 72.7% specific for loosening. Overall intra- and interobserver reliability was highly variable, but zonal analysis showed that lucency at the tip of the femoral peg was significantly associated with loosening of the femoral component. Fluoroscopically guided radiographs may improve the zonal reliability of the assessment of RLLs, but further independent and comparative studies are required. In the meantime, the innocence of the physiological RLLs detected by standard radiographs should be viewed with caution.
The aim of our study was to describe the characteristics,
treatment, and outcomes of patients with periprosthetic joint infection
(PJI) and normal inflammatory markers after total knee arthroplasty
(TKA) and total hip arthroplasty (THA). In total 538 TKAs and 414 THAs underwent surgical treatment for
PJI and met the inclusion criteria. Pre-operative erythrocyte sedimentation
rate (ESR) and C-reactive protein level (CRP) were reviewed to identify
the seronegative cohort. An age- and gender-matched cohort was identified
from the remaining patients for comparison. Overall, 4% of confirmed
infections were seronegative (21 TKA and 17 THA). Of those who underwent
pre-operative aspiration, cultures were positive in 76% of TKAs
(n = 13) and 64% of THAs (n = 7). Cell count and differential were
suggestive of infection in 85% of TKA (n = 11) and all THA aspirates

Revision knee arthroplasty presents a number
of challenges, not least of which is obtaining solid primary fixation
of implants into host bone. Three anatomical zones exist within
both femur and tibia which can be used to support revision implants.
These consist of the joint surface or epiphysis, the metaphysis
and the diaphysis. The methods by which fixation in each zone can
be obtained are discussed. The authors suggest that solid fixation
should be obtained in at least two of the three zones and emphasise
the importance of pre-operative planning and implant selection. Cite this article:
The purpose of this study was to examine the effect of posterior
cruciate ligament (PCL) retention, PCL recession, and PCL excision
during cruciate-retaining total knee replacement. A total of 3018 anatomic graduated component total knee replacements
were examined; 1846 of these retained the PCL, 455 PCLs were partially
recessed, and in 717 the PCL was completely excised from the back
of the tibia.Objectives

Bone defects are occasionally encountered during
primary total knee replacement (TKR) and cause difficulty in establishing
a stable well-aligned bone-implant interface. Between March 1999
and November 2005, 59 knees in 43 patients underwent primary TKR
with a metal block augmentation for tibial bone deficiency. In all,
six patients (eight knees) died less than four years post-operatively,
and four patients (five knees) were lost to follow-up leaving 46 knees
in 33 patients available for review at a mean of 78.6 months (62
to 129). The clinical results obtained, including range of movement,
American Knee Society and Oxford knee scores, and the Western Ontario
and McMaster Universities osteoarthritis index, were good to excellent,
with no failures. Radiolucent lines at the block-cement-bone interface
were noted in five knees (11%) during the first post-operative year,
but these did not progress. Modular rectangular metal augmentation for tibial bone deficiency
is a useful option. No deterioration of the block-prosthesis or
block-cement-bone interface was seen at minimum of five years follow-up.

We studied the intra- and interobserver reliability of measurements of the position of the components after total knee replacement (TKR) using a combination of radiographs and axial two-dimensional (2D) and three-dimensional (3D) reconstructed CT images to identify which method is best for this purpose. A total of 30 knees after primary TKR were assessed by two independent observers (an orthopaedic surgeon and a radiologist) using radiographs and CT scans. Plain radiographs were highly reliable at measuring the tibial slope, but showed wide variability for all other measurements; 2D-CT also showed wide variability. 3D-CT was highly reliable, even when measuring rotation of the femoral components, and significantly better than 2D-CT. Interobserver variability in the measurements on radiographs were good (intraclass correlation coefficient (ICC) 0.65 to 0.82), but rotational measurements on 2D-CT were poor (ICC 0.29). On 3D-CT they were near perfect (ICC 0.89 to 0.99), and significantly more reliable than 2D-CT (p <
0.001). 3D-reconstructed images are sufficiently reliable to enable reporting of the position and orientation of the components. Rotational measurements in particular should be performed on 3D-reconstructed CT images. When faced with a poorly functioning TKR with concerns over component positioning, we recommend 3D-CT as the investigation of choice.
The lateral compartment is predominantly affected
in approximately 10% of patients with osteoarthritis of the knee. The
anatomy, kinematics and loading during movement differ considerably
between medial and lateral compartments of the knee. This in the
main explains the relative protection of the lateral compartment
compared with the medial compartment in the development of osteoarthritis.
The aetiology of lateral compartment osteoarthritis can be idiopathic,
usually affecting the femur, or secondary to trauma commonly affecting
the tibia. Surgical management of lateral compartment osteoarthritis
can include osteotomy, unicompartmental knee replacement and total
knee replacement. This review discusses the biomechanics, pathogenesis

Revision total knee arthroplasty (TKA) is a complex
procedure which carries both a greater risk for patients and greater
cost for the treating hospital than does a primary TKA. As well
as the increased cost of peri-operative investigations, blood transfusions,
surgical instrumentation, implants and operating time, there is
a well-documented increased length of stay which accounts for most
of the actual costs associated with surgery. We compared revision surgery for infection with revision for
other causes (pain, instability, aseptic loosening and fracture).
Complete clinical, demographic and economic data were obtained for
168 consecutive revision TKAs performed at a tertiary referral centre
between 2005 and 2012. Revision surgery for infection was associated with a mean length
of stay more than double that of aseptic cases (21.5 Current NHS tariffs do not fully reimburse the increased costs
of providing a revision knee surgery service. Moreover, especially
as greater costs are incurred for infected cases. These losses may
adversely affect the provision of revision surgery in the NHS. Cite this article:
Radiological evidence of post-traumatic osteoarthritis
(PTOA) after fracture of the tibial plateau is common but end-stage arthritis
which requires total knee arthroplasty is much rarer. The aim of this study was to examine the indications for, and
outcomes of, total knee arthroplasty after fracture of the tibial
plateau and to compare this with an age and gender-matched cohort
of TKAs carried out for primary osteoarthritis. Between 1997 and 2011, 31 consecutive patients (23 women, eight
men) with a mean age of 65 years (40 to 89) underwent TKA at a mean
of 24 months (2 to 124) after a fracture of the tibial plateau.
Of these, 24 had undergone ORIF and seven had been treated non-operatively.
Patients were assessed pre-operatively and at 6, 12 and >
60 months
using the Short Form-12, Oxford Knee Score and a patient satisfaction
score. Patients with instability or nonunion needed total knee arthroplasty
earlier (14 and 13.3 months post-injury) than those with intra-articular
malunion (50 months, p <
0.001). Primary cruciate-retaining implants
were used in 27 (87%) patients. Complication rates were higher in

Although it has been suggested that the outcome
after revision of a unicondylar knee replacement (UKR) to total knee
replacement (TKR) is better when the mechanism of failure is understood,
a comparative study on this subject has not been undertaken. A total of 30 patients (30 knees) who underwent revision of their
unsatisfactory UKR to TKR were included in the study: 15 patients
with unexplained pain comprised group A and 15 patients with a defined
cause for pain formed group B. The Oxford knee score (OKS), visual
analogue scale for pain (VAS) and patient satisfaction were assessed before
revision and at one year after revision, and compared between the
groups. The mean OKS improved from 19 (10 to 30) to 25 (11 to 41) in
group A and from 23 (11 to 45) to 38 (20 to 48) in group B. The
mean VAS improved from 7.7 (5 to 10) to 5.4 (1 to 8) in group A
and from 7.4 (2 to 9) to 1.7 (0 to 8) in group B. There was a statistically
significant difference between the mean improvements in each group
for both OKS (p = 0.022) and VAS (p = 0.002). Subgroup analysis
in group A, performed in order to define a patient factor that predicts
outcome of revision surgery in patients with unexplained pain, showed
no pre-operative differences between both subgroups. These results may be used to inform patients about what to expect
from revision surgery, highlighting that revision of UKR to TKR
for unexplained pain generally results in a less favourable outcome

The use of hinged implants in primary total knee
replacement (TKR) should be restricted to selected indications and mainly
for elderly patients. Potential indications for a rotating hinge
or pure hinge implant in primary TKR include: collateral ligament
insufficiency, severe varus or valgus deformity (>
20°) with necessary
relevant soft-tissue release, relevant bone loss including insertions
of collateral ligaments, gross flexion-extension gap imbalance,
ankylosis, or hyperlaxity. Although data reported in the literature
are inconsistent, clinical results depend on implant design, proper
technical use, and adequate indications. We present our experience
with a specific implant type that we have used for over 30 years
and which has given our elderly patients good mid-term results.
Because revision of implants with long cemented stems can be very
challenging, an effort should be made in the future to use shorter stems
in modular versions of hinged implants. Cite this article:
Two-stage revision surgery for infected total knee replacement offers the highest rate of success for the elimination of infection. The use of articulating antibiotic-laden cement spacers during the first stage to eradicate infection also allows protection of the soft tissues against excessive scarring and stiffness. We have investigated the effect of cyclical loading of cement spacers on the elution of antibiotics. Femoral and tibial spacers containing vancomycin at a constant concentration and tobramycin of varying concentrations were studied The elution of tobramycin increased proportionately with its concentration in cement and was significantly higher at all sampling times from five minutes to 1680 minutes in loaded components compared with the control group (p = 0.021 and p = 0.003, respectively). A similar trend was observed with elution of vancomycin, but this failed to reach statistical significance at five, 1320 and 1560 minutes (p = 0.0508, p = 0.067 and p = 0.347, respectively). However, cyclically loaded and control components showed an increased elution of vancomycin with increasing tobramycin concentration in the specimens, despite all components having the same vancomycin concentration. The concentration of tobramycin influences both tobramycin and vancomycin elution from bone cement. Cyclical loading of the cement spacers enhanced the elution of vancomycin and tobramycin.
Progressive degenerative changes in the medial
compartment of the knee following lateral unicompartmental arthroplasty
(UKA) remains a leading indication for revision surgery. The purpose
of this study is to evaluate changes in the congruence and joint
space width (JSW) of the medial compartment following lateral UKA.
The congruence of the medial compartment of 53 knees (24 men, 23
women, mean age 13.1 years; Our data suggest that a well conducted lateral UKA may improve
the congruence and normalise the JSW of the medial compartment,
potentially preventing progression of degenerative change. Cite this article:
Structural allografts may be used to manage uncontained
bone defects in revision total knee replacement (TKR). However,
the availability of cadaver grafts is limited in some areas of Asia.
The aim of this study was to evaluate the mid-term outcome of the
use of femoral head allografts for the reconstruction of uncontained
defects in revision TKR, focusing on complications related to the
graft. We retrospectively reviewed 28 patients (30 TKRs) with Anderson
Orthopaedic Research Institute (AORI) type 3 bone defects, who underwent
revision using femoral head allografts and stemmed components. The
mean number of femoral heads used was 1.7 (1 to 3). The allograft–host
junctions were packed with cancellous autograft. At a mean follow-up of 76 months (38 to 136) the mean American
Knee Society knee score improved from 37.2 (17 to 60) pre-operatively
to 90 (83 to 100) (p <
0.001). The mean function score improved
from 26.5 (0 to 50) pre-operatively to 81 (60 to 100) (p <
0.001).
All the grafts healed to the host bone. The mean time to healing
of the graft was 6.6 months
(4 to 16). There have been no complications of collapse of the graft,
nonunion, infection or implant loosening. No revision surgery was
required. The use of femoral head allografts in conjunction with a stemmed
component and autogenous bone graft in revision TKR in patients
with uncontained bone defects resulted in a high rate of healing
of the graft with minimal complications and a satisfactory outcome.
Longer follow-up is needed to observe the evolution of the graft. Cite this article:
We investigated whether the extension gap in total knee replacement (TKR) would be changed when the femoral component was inserted. The extension gap was measured with and without the femoral component in place in 80 patients with varus osteoarthritis undergoing posterior-stabilised TKR. The effect of a post-operative increase in the size of the femoral posterior condyles was also evaluated. The results showed that placement of the femoral component significantly reduced the medial and lateral extension gaps by means of 1.0 mm and 0.9 mm, respectively (p <
0.0001). The extension gap was reduced when a larger femoral component was selected relative to the thickness of the resected posterior condyle. When the post-operative posterior lateral condyle was larger than that pre-operatively, 17 of 41 knees (41%) showed a decrease in the extension gap of >
2.0 mm. When a specially made femoral trial component with a posterior condyle enlarged by 4 mm was tested, the medial and lateral extension gaps decreased further by means of 2.1 mm and 2.8 mm, respectively. If the thickness of the posterior condyle is expected to be larger than that pre-operatively, it should be recognised that the extension gap is likely to be altered. This should be taken into consideration when preparing the extension gap.
