This follows several national joint registry studies documenting high UKA revision rates when compared to total knee arthroplasty (TKA). With the recent introduction of robotically assisted UKA procedures, it is hoped that outcomes improve. This study examines the cumulative revision rate of UKA procedures implanted with a newly introduced robotic system and compares the results to one of the best performing non-robotically assisted UKA prostheses, as well as all other non-robotically assisted UKA procedures. Data from the Australian Orthopaedic Association National Joint Arthroplasty Registry (AOANJRR) for all UKA procedures performed for osteoarthritis (OA) between 2015 and 2018 were analyzed. A pragmatic, single-centre, double-blind randomized clinical trial was conducted in a NHS teaching hospital to evaluate whether there is a difference in functional knee scores, quality-of-life outcome assessments, and complications at one-year after intervention between total knee arthroplasty (TKA) and patellofemoral arthroplasty (PFA) in patients with severe isolated patellofemoral arthritis. This parallel, two-arm, superiority trial was powered at 80%, and involved 64 patients with severe isolated patellofemoral arthritis. The primary outcome measure was the functional section of the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) score at 12 months. Secondary outcomes were the full 24-item WOMAC, Oxford Knee Score (OKS), American Knee Society Score (AKSS), EuroQol five dimension (EQ-5D) quality-of-life score, the University of California, Los Angeles (UCLA) Physical Activity Rating Scale, and complication rates collected at three, six, and 12 months. Although internal hemipelvectomy is associated with a high incidence of morbidity, especially wound complications, few studies have examined rates of wound complications in these patients or have identified factors associated with the consequences. The present study aimed to: 1) determine the rate of wound and other complications requiring surgery after internal hemipelvectomy; and 2) identify factors that affect the rate of wound complications and can be used to stratify patients by risk of wound complications. The aim of this study is to evaluate the clinical results of operative intervention for femoral metastases which were selected based on expected survival and to discuss appropriate surgical strategies. From 2002 to 2017, 148 consecutive patients undergoing surgery for femoral metastasis were included in this study. Prognostic risk assessments were performed according to the Katagiri and revised Katagiri scoring system. In general, the low-risk group underwent resection and reconstruction with endoprosthetic replacement (EPR), while the high-risk group underwent internal fixation (IF) and radiation therapy. For the intermediate-risk group, the operative choice depended on the patient’s condition, degree of bone destruction, and radio-sensitivity. femoral fractures in 32 644 primary total hip arthroplasties (THAs). . Results. There were 564 intra-operative fractures (1.7%); 529 during uncemented
stem placement (3.0%) and 35 during cemented stem placement (0.23%).
Intra-operative fractures were more common in females and patients

of the femoral component (60%), and involved the calcar (69%). There
were 557 post-operative fractures (20-year probability: 3.5%; 95%
confidence interval (CI) 3.2 to 3.9); 335 fractures after placement
of an uncemented stem (20-year probability: 7.7%; 95% CI 6.2 to
9.1) and 222 after placement of a cemented stem (20-year probability:
2.1%; 95% CI 1.8 to 2.5). The probability of a post-operative fracture
within 30 days after an uncemented stem was ten times higher than
a cemented stem. The most common post-operative fracture type was

Aims. We reviewed all patients who sustained a fracture of the hip
and were treated in Northern Ireland over a period of 15 years to
identify trends in incidence, the demographics of the patients,
the rates of mortality, the configuration of the fracture and the
choice of implant. Patients and Methods. Since 01 January 2001 data about every fracture of the hip sustained
in an adult have been collected centrally in Northern Ireland. All
adults with such a fracture between 2000 and 2015 were included
in the study. Temporal changes in their demographics, the mode of
treatment, and outcomes including mortality were analysed. Results. The incidence of fractures of the hip, in Northern Ireland, rose
from 54 in 100 000 in 2000 to 86 in 100 000 in 2015. If these trends
continue, we predict this rising to 128 in 100 000 in 2030. We found
that these patients are becoming older and increasingly frail, as
assessed by the American Association of Anesthesiology grade. Complex extracapsular
fractures have become more common since 2009, which may explain
the increased use of cephalomedullary nails. Despite increasing
frailty, the 30-day and 12-month rates of mortality fell significantly

(TKA) often present with a recurrent effusion, which may be a haemarthrosis.
While the radiographic factors contributing to flexion instability
have been elucidated, the clinical diagnosis remains challenging.
Our aim, in this study, was to determine the mean white cell count
and differential profile in pre-operative aspirations of synovial
fluid in a consecutive series of patients undergoing revision TKA
for flexion instability. Patients and Methods. Between 2000 and 2010, 60 patients undergoing aseptic revision
TKA for flexion instability were identified. The results of the
pre-operative aspiration of synovial fluid were available for 53
patients (88%). These patients were 1:2 matched to 106 patients
who underwent aseptic TKA for indications other than flexion instability.

Robotic-assisted total knee arthroplasty (RA-TKA) has been introduced to provide accurate bone cuts and help achieve the target knee alignment, along with symmetric gap balancing. The purpose of this study was to determine if any early clinical benefits could be realized following TKA using robotic-assisted technology. A lack of supporting clinical studies have been published to determine the ideal length of intramedullary nail in fixation of trochanteric fractures of the hip. Nevertheless, there has been a trend to use shorter intramedullary nails for the internal fixation of trochanteric hip fractures. Our aim was to determine if the length of nail affected the outcome. We randomized 229 patients with a trochanteric hip fracture between two implants: a ‘standard’ nail of 220 mm and a shorter nail of 175 mm, which had decreased proximal angulation (4° vs 7°) and a reduced diameter at the level of the lesser trochanter. Patients were followed up for one year by a nurse blinded to the type of implant used to determine if there were differences in mobility and pain with two nail designs. Pain was assessed on a scale of 1 (none) to 8 (severe and constant) and mobility on a scale of 1 (full mobility) to 9 (immobile).Aims
