The best surgical strategy for the management of displaced bucket-handle (BH) meniscal tears in an anterior cruciate ligament (ACL)-deficient knee is unclear. Combining meniscal repair with ACL reconstruction (ACLR) is thought to improve meniscal healing rates; however, patients with displaced BH meniscal tears may lack extension. This leads some to advocate staged surgery to avoid postoperative stiffness and loss of range of motion (ROM) following ACLR. We reviewed the data for a consecutive series of 88 patients (mean age 27.1 years (15 to 49); 65 male (74%) and 23 female (26%)) who underwent single-stage repair of a displaced BH meniscal tear (67 medial (76%) and 21 lateral (24%)) with concomitant hamstring autograft ACLR. The patient-reported outcome measures (PROMs) EuroQol visual analogue scale (EQ-VAS), EuroQol five-dimension health questionnaire (EQ-5D), Knee injury and Osteoarthritis Outcome Score (KOOS), International Knee Documentation Committee score (IKDC), and Tegner score were recorded at final follow-up. The mainstay of surgical correction of hallux valgus is first
metatarsal osteotomy, either proximally or distally. We present
a technique of combining a distal chevron osteotomy with a proximal
opening wedge osteotomy, for the correction of moderate to severe
hallux valgus. We reviewed 45 patients (49 feet) who had undergone double osteotomy.
Outcome was assessed using the American Orthopaedic Foot and Ankle
Society (AOFAS) and the Short Form (SF) -36 Health Survey scores.
Radiological measurements were undertaken to assess the correction. The mean age of the patients was 60.8 years (44.2 to 75.3). The

Patients and Methods
Haemophilia is an x-linked inherited bleeding disorder which can cause severe arthropathy. We have reviewed the results of 70 primary total knee replacements (TKR) performed in 57 haemophilic patients between 1983 and 2007. The functional results were assessed using the Hospital for Special Surgery (HSS) knee scoring system and Kaplan-Meier survivorship analysis. Six patients died. HSS scores were available for 60 TKRs at a mean follow-up of 9.2 years (2 to 23); 57 (95%) had good or excellent results. Deep infection was recorded in one patient. Kaplan-Meier analysis using infection and aseptic loosening as endpoints showed the survival rate at 20 years to be 94.0%. A reduction in infection, spontaneous haemarthrosis and improvement in the quality of life were noted to justify surgery in our series of patients with a mean age of 43 (25 to 70). We have found that using the latest techniques of continuous infusion of clotting Factor have significantly helped to reduce the complication rates and have achieved results which match those of the non-haemophilic population undergoing TKR.

A method of treatment of displaced supracondylar fractures of the humerus in children by manipulative reduction and fixation in plaster in full extension of the elbow and supination of the forearm is described. 2. The method is easy, safe and requires a short period of hospitalisation. The carrying angle at the elbow can only be recorded, controlled and maintained when the elbow is extended and the forearm is fully supinated. Thus cubitus varus can be avoided. 3. The results of treatment in seventy-two displaced fractures treated by this method are reported. 4. Treatment by other methods is reviewed.

Randomized, controlled, clinical trials that compared autologous and synthetic bone grafts in tibial plateau fractures were included. Patients with a deformity of the hindfoot present a particular challenge when performing total knee arthroplasty (TKA). The literature contains little information about the relationship between TKA and hindfoot alignment. This systematic review aimed to determine from both clinical and radiological studies whether TKA would alter a preoperative hindfoot deformity and whether the outcome of TKA is affected by the presence of a postoperative hindfoot deformity. A systematic literature search was performed in the databases PubMed, EMBASE, Cochrane Library, and Web of Science. Search terms consisted of “total knee arthroplasty/replacement” combined with “hindfoot/ankle alignment”. Inclusion criteria were all English language studies analyzing the association between TKA and the alignment of the hindfoot, including the clinical or radiological outcomes. Exclusion criteria consisted of TKA performed with a concomitant extra-articular osteotomy and case reports or expert opinions. An assessment of quality was conducted using the modified Methodological Index for Non-Randomized Studies (MINORS). The review was performed according to the Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA) guidelines and registered in the PROSPERO database (CRD42019106980).Aims
Methods
We report a long-term follow-up of abduction-extension osteotomy of the first metacarpal, performed for painful trapeziometacarpal osteoarthritis. Of a consecutive series of 50 operations, 41 thumbs (82%) were reviewed at a mean follow-up of 6.8 years. Good or excellent pain relief was achieved in 80%, and 93% considered that surgery had improved hand function, while 82% had normal grip and pinch strength, with restoration of thumb abduction. Metacarpal osteotomy was equally successful in relieving symptoms of those with early (grade 2) and moderate (grade 3) degenerative changes. This simple procedure provides lasting pain relief, corrects adduction contracture and restores grip and pinch strength, giving good results with few complications.
1. The results have been reviewed of forty-two traumatic dislocations of the knee in forty-one patients, twenty-six of which were treated conservatively. 2. Primary operative repair was carried out in only three cases, although a further ten had operative intervention for various complications. 3. It was not possible to compare the results of operation with those of conservative treatment, because the operative procedures were so diverse in nature. 4. The conservative treatment of twenty-six dislocations yielded surprisingly good results with regard to stability, absence of pain, and range of flexion movement. 5. In general, immobilisation for long periods, like operative repair, led to reduced movement. 6. In the absence of complications, conservative treatment is the method of choice.
1. In two dogs, approximately one to two years and three to four months of age, an experimental comparison was made between the calcium accretion rate as defined by the Bauer-Carlsson-Lindquist equation, and the bone formation rate determined by double tetracycline labelling. 2. The overall calcium accretion rate was determined from the specific activity of the blood plasma, and the urinary and faecal excretion of isotope, following an intravenous tracer dose of Ca45. A time of five days after injection was used for the calculation of accretion rates, but data for shorter times of calculation are included. 3. Local accretion rates were obtained for different parts of the skeleton by determining the specific activities of bone samples at the end of the experiment. 4. The amount of isotope the uptake of which was not related to new bone formation (the diffuse component) was determined autoradiographically. 5. Local values for appositional growth rate and bone formation rate were obtained, using sections of undecalcified bone specimens, by measuring the linear separation between two tetracycline bone markers and the area of new bone enclosed by them. 6. In the older dog, the measurements for cortical bone showed that the accretion rate was two to three times as great as the bone formation rate: the observed diffuse component was sufficient to account for the greater part of this difference. Measurement of the bone formation rate for cancellous bone presented difficulties, but the approximate values obtained suggested that the accretion rate and the bone formation rate were of about the same order for this tissue. 7. In the younger dog, the bone formation rate could be determined only in cortical bone: at the sites studied, the values for the accretion rate and the bone formation rate did not differ by more than 20 per cent. It is suggested that this is due partly to the low specific activity of the diffuse component in this young animal, and partly to the relatively large amounts of new bone formed during the period of the experiment. 8. Despite the important differences between the rates of calcium accretion and bone formation that were found to exist in regions where there was only a small amount of new bone formation, there was a strong correlation between the two rates. The value of the accretion rate as a parameter of bone metabolism is clear.
1. Details of clinical, radiological, biochemical and histological investigations of a case of metaphysial dysostosis are presented. 2. The patient was a boy of seven years, and showed widespread lesions (involving long bones of limbs, small bones of hands and feet, pelvis, clavicles, and ribs) characterised by retardation of growth and ossification with masses of partly calcified tissue in the metaphyses. 3. No radiological, clinical or histological evidence of renal osteodystrophy was found in the case described. 4. Metaphysial dysostosis is discussed in relation to other chondrodystrophies.
Two cases of benign osteoblastic tumours of the spine, occurring in young patients, and presenting as expanding osteolytic lesions with some central calcification and ossification, are described. It is suggested that they represent a condition allied to, but usefully separated from, osteoid osteoma of bone. The name osteogenic fibroma of bone, proposed by Lichtenstein for this type of case, is accepted.
Progressive degenerative changes in the medial
compartment of the knee following lateral unicompartmental arthroplasty
(UKA) remains a leading indication for revision surgery. The purpose
of this study is to evaluate changes in the congruence and joint
space width (JSW) of the medial compartment following lateral UKA.
The congruence of the medial compartment of 53 knees (24 men, 23
women, mean age 13.1 years; Our data suggest that a well conducted lateral UKA may improve
the congruence and normalise the JSW of the medial compartment,

We present our experience of forearm lengthening
in children with various conditions performed by a single surgeon between
1995 and 2009. A total of 19 children with a mean age of 9.8 years
(2.1 to 15.9) at the time of surgery had 22 forearm lengthenings
using either an Ilizarov/spatial and Ilizarov circular frame or
a monolateral external fixator. The patients were divided into two
groups: group A, in whom the purpose of treatment was to restore
the relationship between the radius and the ulna, and group B, in
whom the objective was to gain forearm length. The mean follow-up after
removal of the frame was 26 months (13 to 53). There were ten patients (11 forearms) in group A with a mean
radioulnar discrepancy of 2.4 cm (1.5 to 3.3) and nine patients
(11 forearms) in group B. In group A, the mean lengthening achieved
was 2.7 cm (1.0 to 5.5), with a lengthening index of 11.1 weeks/cm.
Equalisation or overcorrection of the discrepancy was achieved in
seven of 11 forearms, but lengthening was only partially successful
at preventing subluxation or dislocation of the radial head. In
group B, the mean lengthening achieved was 3.8 cm (1.9 to 6.8),
with a lengthening index of 7.25 weeks/cm. Common complications
in both groups were pin-site infection and poor regenerate formation. Forearm lengthening by distraction osteogenesis is a worthwhile
procedure in children that can improve cosmesis and function, particularly
in patients with shortening of both radius and ulna.
