The lack of effective treatment for cartilage defects has prompted investigations using tissue engineering techniques for their regeneration and repair. The success of tissue-engineered repair of cartilage may depend on the rapid and efficient adhesion of transplanted cells to a scaffold. Our aim in this study was to repair full-thickness defects in articular cartilage in the weight-bearing area of a porcine model, and to investigate whether the CD44 monoclonal antibody biotin-avidin (CBA) binding technique could provide satisfactory tissue-engineered cartilage. Cartilage defects were created in the load-bearing region of the lateral femoral condyle of mini-type pigs. The defects were repaired with traditional tissue-engineered cartilage, tissue-engineered cartilage constructed with the biotin-avidin (BA) technique, tissue-engineered cartilage constructed with the CBA technique and with autologous cartilage. Our aim was to examine the clinical and radiographic outcomes
in 257 consecutive Oxford unicompartmental knee arthroplasties (OUKAs)
(238 patients), five years post-operatively. A retrospective evaluation was undertaken of patients treated
between April 2008 and October 2010 in a regional centre by two
non-designing surgeons with no previous experience of UKAs. The
Oxford Knee Scores (OKSs) were recorded and fluoroscopically aligned

Methods
Osteochondritis Dissecans (OCD) is a condition
for which the aetiology remains unknown. It affects subchondral bone
and secondarily its overlying cartilage and is mostly found in the
knee. It can occur in adults, but is generally identified when growth
remains, when it is referred to as juvenile OCD. As the condition
progresses, the affected subchondral bone separates from adjacent
healthy bone, and can lead to demarcation and separation of its associated
articular cartilage. Any symptoms which arise relate to the stage
of the disease. Early disease without separation of the lesion results
in pain. Separation of the lesion leads to mechanical symptoms and
swelling and, in advanced cases, the formation of loose bodies. Early identification of OCD is essential as untreated OCD can
lead to the premature degeneration of the joint, whereas appropriate
treatment can halt the disease process and lead to healing. Establishing
the stability of the lesion is a key part of providing the correct
treatment. Stable lesions, particularly in juvenile patients, have
greater propensity to heal with non-surgical treatment, whereas
unstable or displaced lesions usually require surgical management. This article discusses the aetiology, clinical presentation and
prognosis of OCD in the knee. It presents an algorithm for treatment,
which aims to promote healing of native hyaline cartilage and to

Knee arthrodesis is a potential salvage procedure
for limb preservation after failure of total knee arthroplasty (TKA) due
to infection. In this study, we evaluated the outcome of single-stage
knee arthrodesis using an intramedullary cemented coupled nail without
bone-on-bone fusion after failed and infected TKA with extensor
mechanism deficiency. Between 2002 and 2012, 27 patients (ten female,
17 male; mean age 68.8 years; 52 to 87) were treated with septic
single-stage exchange. Mean follow-up duration was 67.1months (24
to 143, n = 27) (minimum follow-up 24 months) and for patients with
a minimum follow-up of five years 104.9 (65 to 143,; n = 13). A
subjective patient evaluation (Short Form (SF)-36) was obtained,
in addition to the Visual Analogue Scale (VAS). The mean VAS score was
1.44 (SD 1.48). At final follow-up, four patients had recurrent
infections after arthrodesis (14.8%). Of these, three patients were
treated with a one-stage arthrodesis nail exchange; one of the three
patients had an aseptic loosening with a third single-stage exchange,
and one patient underwent knee amputation for uncontrolled sepsis at
108 months. All patients, including the amputee, indicated that
they would choose arthrodesis again. Data indicate that a single-stage
knee arthrodesis offers an acceptable salvage procedure after failed

Patellofemoral complications are common after
total knee replacement (TKR). Leaving the patellar unsurfaced after
TKR may lead to complications such as anterior knee pain, and re-operation
to surface it. Complications after patellar resurfacing include
patellar fracture, aseptic loosening, patellar instability, polyethylene
wear, patellar clunk and osteonecrosis. Historically, patellar complications
account for one of the larger proportions of causes of failure in
TKR, however, with contemporary implant designs, complication rates
have decreased. Most remaining failures relate to patellofemoral
tracking. Understanding the causes of patellofemoral maltracking
is essential to prevent these complications as well as manage them

Cartilage repair in terms of replacement, or
regeneration of damaged or diseased articular cartilage with functional tissue,
is the ‘holy grail’ of joint surgery. A wide spectrum of strategies
for cartilage repair currently exists and several of these techniques
have been reported to be associated with successful clinical outcomes
for appropriately selected indications. However, based on respective
advantages, disadvantages, and limitations, no single strategy, or
even combination of strategies, provides surgeons with viable options
for attaining successful long-term outcomes in the majority of patients.
As such, development of novel techniques and optimisation of current techniques
need to be, and are, the focus of a great deal of research from
the basic science level to clinical trials. Translational research
that bridges scientific discoveries to clinical application involves
the use of animal models in order to assess safety and efficacy

The goal of this study was to determine whether intra-articular
administration of the potentially anti-fibrotic agent decorin influences
the expression of genes involved in the fibrotic cascade, and ultimately
leads to less contracture, in an animal model. A total of 18 rabbits underwent an operation on their right knees
to form contractures. Six limbs in group 1 received four intra-articular
injections of decorin; six limbs in group 2 received four intra-articular
injections of bovine serum albumin (BSA) over eight days; six limbs
in group 3 received no injections. The contracted limbs of rabbits
in group 1 were biomechanically and genetically compared with the
contracted limbs of rabbits in groups 2 and 3, with the use of a
calibrated joint measuring device and custom microarray, respectively.Objectives

Satisfactory primary wound healing following
total joint replacement is essential. Wound healing problems can
have devastating consequences for patients. Assessment of their healing
capacity is useful in predicting complications. Local factors that
influence wound healing include multiple previous incisions, extensive
scarring, lymphoedema, and poor vascular perfusion. Systemic factors
include diabetes mellitus, inflammatory arthropathy, renal or liver
disease, immune compromise, corticosteroid therapy, smoking, and
poor nutrition. Modifications in the surgical technique are necessary
in selected cases to minimise potential wound complications. Prompt
and systematic intervention is necessary to address any wound healing
problems to reduce the risks of infection and other potential complications. Cite this article:
A soft-tissue defect over an infected total knee
replacement (TKR) presents a difficult technical problem that can
be treated with a gastrocnemius flap, which is rotated over the
defect during the first-stage of a revision procedure. This facilitates
wound healing and the safe introduction of a prosthesis at the second
stage. We describe the outcome at a mean follow-up of 4.5 years
(1 to 10) in 24 patients with an infected TKR who underwent this procedure.
A total of 22 (92%) eventually obtained a satisfactory result. The
mean Knee Society score improved from 53 pre-operatively to 103
at the latest follow-up (p <
0.001). The mean Western Ontario
and McMaster Universities osteoarthritis index and Short-Form 12
score also improved significantly (p <
0.001). This form of treatment can be used reliably and safely to treat
many of these complex cases where control of infection, retention
of the components and acceptable functional recovery are the primary

The use of platelet-rich plasma (PRP) as an adjuvant
to tissue repair is gaining favour in orthopaedic surgery. Tunnel widening
after anterior cruciate ligament (ACL) reconstruction is a recognised
phenomenon that could compromise revision surgery. The purpose of
this study was to determine whether PRP might prevent tunnel widening
in ACL reconstruction. Patients undergoing ACL reconstruction using a hamstring graft
were randomly allocated either to have PRP introduced into the tunnels
peri-operatively or not. CT scanning of the knees was carried out
on the day after surgery and at three months post-operatively and
the width of the tunnels was measured. Patients were also evaluated
clinically at three months, when laxity was also measured. Each group comprised 25 patients, and at three months post-operatively
all were pain-free with stable knees, a negative Lachman test and
a good range of movement. Arthrometric results had improved significantly
in both groups (p <
0.001). Despite slightly less tunnel widening
in the PRP group, there was no significant difference between the
groups at the femoral opening or the mid-tunnel (p = 0.370 and p
= 0.363, respectively) nor at the tibial opening or mid-tunnel (p
= 0.333 and p = 0.177, respectively). We conclude that PRP has no significant effect in preventing

Disruption of the extensor mechanism in total
knee arthroplasty may occur by tubercle avulsion, patellar or quadriceps
tendon rupture, or patella fracture, and whether occurring intra-operatively
or post-operatively can be difficult to manage and is associated
with a significant rate of failure and associated complications.
This surgery is frequently performed in compromised tissues, and
repairs must frequently be protected with cerclage wiring and/or
augmentation with local tendon (semi-tendinosis, gracilis) which
may also be used to treat soft-tissue loss in the face of chronic
disruption. Quadriceps rupture may be treated with conservative
therapy if the patient retains active extension. Component loosening
or loss of active extension of 20° or greater are clear indications
for surgical treatment of patellar fracture. Acute patellar tendon
disruption may be treated by primary repair. Chronic extensor failure
is often complicated by tissue loss and retraction can be treated
with medial gastrocnemius flaps, achilles tendon allografts, and
complete extensor mechanism allografts. Attention to fixing the
graft in full extension is mandatory to prevent severe extensor
lag as the graft stretches out over time.
Fixed flexion deformities are common in osteoarthritic
knees that are indicated for total knee arthroplasty. The lack of
full extension at the knee results in a greater force of quadriceps
contracture and energy expenditure. It also results in slower walking
velocity and abnormal gait mechanics, overloading the contralateral
limb. Residual flexion contractures after TKA have been associated
with poorer functional scores and outcomes. Although some flexion contractures may resolve with time after
surgery, a substantial percentage will become permanent. Therefore,
it is essential to correct fixed flexion deformities at the time
of TKA, and be vigilant in the post-operative course to maintain
the correction. Surgical techniques to address pre-operative flexion contractures
include: adequate bone resection, ligament releases, removal of
posterior osteophytes, and posterior capsular releases. Post-operatively,
extension can be maintained with focused physiotherapy, a specially
modified continuous passive motion machine, a contralateral heel
lift, and splinting.
This review considers the surgical treatment
of displaced fractures involving the knee in elderly, osteoporotic patients.
The goals of treatment include pain control, early mobilisation,
avoidance of complications and minimising the need for further surgery.
Open reduction and internal fixation (ORIF) frequently results in
loss of reduction, which can result in post-traumatic arthritis
and the occasional conversion to total knee replacement (TKR). TKR
after failed internal fixation is challenging, with modest functional
outcomes and high complication rates. TKR undertaken as treatment
of the initial fracture has better results to late TKR, but does
not match the outcome of primary TKR without complications. Given
the relatively infrequent need for late TKR following failed fixation,
ORIF is the preferred management for most cases. Early TKR can be
considered for those patients with pre-existing arthritis, bicondylar
femoral fractures, those who would be unable to comply with weight-bearing restrictions,
or where a single definitive procedure is required.
We retrospectively reviewed the records of 1150
computer-assisted total knee replacements and analysed the clinical
and radiological outcomes of 45 knees that had arthritis with a
pre-operative recurvatum deformity. The mean pre-operative hyperextension
deformity of 11° (6° to 15°), as measured by navigation at the start
of the operation, improved to a mean flexion deformity of 3.1° (0°
to 7°) post-operatively. A total of 41 knees (91%) were managed
using inserts ≤ 12.5 mm thick, and none had mediolateral laxity
>
2 mm from a mechanical axis of 0° at the end of the surgery. At
a mean follow-up of 26.4 months (13 to 48) there was significant
improvement in the mean Knee Society, Oxford knee and Western Ontario
and McMaster Universities Osteoarthritis Index scores compared with
the pre-operative values. The mean knee flexion improved from 105°
(80° to 125°) pre-operatively to 131° (120° to 145°), and none of
the limbs had recurrent recurvatum. These early results show that total knee replacement using computer
navigation and an algorithmic approach for arthritic knees with
a recurvatum deformity can give excellent radiological and functional
outcomes without recurrent deformity.
Autologous chondrocyte implantation (ACI) and
mosaicplasty are methods of treating symptomatic articular cartilage
defects in the knee. This study represents the first long-term randomised
comparison of the two techniques in 100 patients at a minimum follow-up
of ten years. The mean age of the patients at the time of surgery was
31.3 years (16 to 49); the mean duration of symptoms pre-operatively
was 7.2 years (9 months to 20 years). The lesions were large with
the mean size for the ACI group being 440.9 mm2 (100
to 1050) and the mosaicplasty group being 399.6 mm2 (100
to 2000). Patients had a mean of 1.5 previous operations (0 to 4)
to the articular cartilage defect. Patients were assessed using
the modified Cincinnati knee score and the Stanmore-Bentley Functional Rating
system. The number of patients whose repair had failed at ten years
was ten of 58 (17%) in the ACI group and 23 of 42 (55%) in the mosaicplasty
group (p <
0.001). The functional outcome of those patients with a surviving graft
was significantly better in patients who underwent ACI compared
with mosaicplasty (p = 0.02).
Correct positioning and alignment of components during primary total knee replacement (TKR) is widely accepted to be an important predictor of patient satisfaction and implant durability. This retrospective study reports the effect of the post-operative mechanical axis of the lower limb in the coronal plane on implant survival following primary TKR. A total of 501 TKRs in 396 patients were divided into an aligned group with a neutral mechanical axis (± 3°) and a malaligned group where the mechanical axis deviated from neutral by >
3°. At 15 years’ follow-up, 33 of 458 (7.2%) TKRs were revised for aseptic loosening. Kaplan-Meier survival analysis showed a weak tendency towards improved survival with restoration of a neutral mechanical axis, but this did not reach statistical significance (p = 0.47). We found that the relationship between survival of a primary TKR and mechanical axis alignment is weaker than that described in a number of previous reports.
Pulmonary embolism is a serious complication after arthroscopy of the knee, about which there is limited information. We have identified the incidence and risk factors for symptomatic pulmonary embolism after arthroscopic procedures on outpatients. The New York State Department of Health Statewide Planning and Research Cooperative System database was used to review arthroscopic procedures of the knee performed on outpatients between 1997 and 2006, and identify those admitted within 90 days of surgery with an associated diagnosis of pulmonary embolism. Potential risk factors included age, gender, complexity of surgery, operating time defined as the total time that the patient was actually in the operating room, history of cancer, comorbidities, and the type of anaesthesia. We identified 374 033 patients who underwent 418 323 outpatient arthroscopies of the knee. There were 117 events of pulmonary embolism (2.8 cases for every 10 000 arthroscopies). Logistic regression analysis showed that age and operating time had significant dose-response increases in risk (p <
0.001) for a subsequent admission with a pulmonary embolism. Female gender was associated with a 1.5-fold increase in risk (p = 0.03), and a history of cancer with a threefold increase (p = 0.05). These risk factors can be used when obtaining informed consent before surgery, to elevate the level of clinical suspicion of pulmonary embolism in patients at risk, and to establish a rationale for prospective studies to test the clinical benefit of thromboprophylaxis in high-risk patients.
Isolated patellofemoral osteoarthritis can be a disabling disease. When conservative treatment fails, surgical options can be unpredictable and may be considered too aggressive for middle-aged and active people. We analysed the clinical and radiological results of a new coronal osteotomy involving thinning of the patella in a selected group of patients with isolated patellofemoral osteoarthritis. Since 1991, 31 patients (35 knees) have been treated, of whom 34 were available for follow-up at a mean of 9.1 years. The Knee Society Score, the Patellar score and the Short-form-36 questionnaire were used for clinical evaluation. We also examined the radiological features to confirm bone consolidation and assess the progression of osteoarthritis. A significant improvement in the functional scores and radiological parameters was noted. All patients except one were satisfied with the operation. Radiological progression of the patellofemoral osteoarthritis was slowed but radiological femorotibial osteoarthritis progressed in 23 (65%) cases, with a total knee replacement becoming necessary in four cases without technical problems in resurfacing the patella. We compared the results with other forms of surgical treatment reported in the literature. This treatment offers good clinical and radiological results, presenting an alternative method of managing patellofemoral osteoarthritis.
