Aims. Tibial plateau fractures are serious injuries about the knee that have the potential to affect patients’ long-term function. To our knowledge, this is the first study to use patient-reported outcomes (PROs) with a musculoskeletal focus to assess the long-term outcome, as compared to a short-term outcome baseline, of tibial plateau fractures treated using modern techniques. Methods. In total, 102 patients who sustained a displaced tibial plateau fracture and underwent operative repair by one of three orthopaedic traumatologists at a large, academic medical centre and had a minimum of five-year follow-up were identified. Acetabular fractures are associated with long-term morbidity. Our prospective cohort study sought to understand the recovery trajectory of this injury over five years. Eligible patients at a level I trauma centre were recruited into a longitudinal registry of surgical acetabular fractures between June 2004 and August 2019. Patient-reported outcome measures (PROMs), including the 36-Item Short Form Health Survey (SF-36) physical component summary (PCS), were recorded at baseline pre-injury recall and six months, one year, two years, and five years postoperatively. We assessed the value of the Clinical Frailty Scale (CFS) in the prediction of adverse outcome after hip fracture. Of 1,577 consecutive patients aged > 65 years with a fragility hip fracture admitted to one institution, for whom there were complete data, 1,255 (72%) were studied. Clinicians assigned CFS scores on admission. Audit personnel routinely prospectively completed the Standardised Audit of Hip Fracture in Europe form, including the following outcomes: 30-day survival; in-hospital complications; length of acute hospital stay; and new institutionalization. The relationship between the CFS scores and outcomes was examined graphically and the visual interpretations were tested statistically. The predictive values of the CFS and Nottingham Hip Fracture Score (NHFS) to predict 30-day mortality were compared using receiver operating characteristic area under the curve (AUC) analysis.Aims
Methods
Crescent fracture dislocations are a well-recognised subset of pelvic ring injuries which result from a lateral compression force. They are characterised by disruption of the sacroiliac joint and extend proximally as a fracture of the posterior iliac wing. We describe a classification with three distinct types. Type I is characterised by a large crescent fragment and the dislocation comprises no more than one-third of the sacroiliac joint, which is typically inferior. Type II fractures are associated with an intermediate-size crescent fragment and the dislocation comprises between one- and two-thirds of the joint. Type III fractures are associated with a small crescent fragment where the dislocation comprises most, but not all of the joint. The principal goals of surgical intervention are the accurate and stable reduction of the sacroiliac joint. This classification proves useful in the selection of both the surgical approach and the reduction technique. Open reduction and plate fixation (ORPF) for displaced proximal humerus fractures can achieve reliably good long-term outcomes. However, a minority of patients have persistent pain and stiffness after surgery and may benefit from open arthrolysis, subacromial decompression, and removal of metalwork (ADROM). The long-term results of ADROM remain unknown; we aimed to assess outcomes of patients undergoing this procedure for stiffness following ORPF, and assess predictors of poor outcome. Between 1998 and 2018, 424 consecutive patients were treated with primary ORPF for proximal humerus fracture. ADROM was offered to symptomatic patients with a healed fracture at six months postoperatively. Patients were followed up retrospectively with demographic data, fracture characteristics, and complications recorded. We reviewed 78 femoral and tibial nonunions treated between January 1992 and December 2003. Of these, we classified 41 in 40 patients as complex cases because of infection (22), bone loss (6) or failed previous surgery (13). The complex cases were all treated with Ilizarov frames. At a mean time of 14.1 months (4 to 38), 39 had healed successfully. Using the Association for the Study and Application of the Methods of Ilizarov scoring system we obtained 17 excellent, 14 good, four fair and six poor bone results. To compare the functionality of adults with displaced mid-shaft clavicular fractures treated either operatively or nonoperatively and to compare the relative risk of nonunion and reoperation between the two groups. Based on specific eligibility criteria, 120 adults (median age 37.5 years (interquartile range (18 to 61)) and 84% males (n = 101)) diagnosed with an acute displaced mid-shaft fracture were recruited, and randomized to either the operative (n = 60) or nonoperative (n = 60) treatment group. This randomized controlled, partially blinded trial followed patients for 12 months following initial treatment. Functionality was assessed by the Constant score (CS) (assessor blinded to treatment) and Disability of the Arm, Shoulder and Hand (DASH) score. Acetabular fractures in older adults lead to a high risk of mortality and morbidity. However, only limited data have been published documenting functional outcomes in such patients. The aims of this study were to describe outcomes in patients aged 60 years and older with operatively managed acetabular fractures, and to establish predictors of conversion to total hip arthroplasty (THA). We conducted a retrospective, registry-based study of 80 patients aged 60 years and older with acetabular fractures treated surgically at The Alfred and Royal Melbourne Hospital. We reviewed charts and radiological investigations and performed patient interviews/examinations and functional outcome scoring. Data were provided by the Victorian Orthopaedic Trauma Outcomes Registry (VOTOR). Survival analysis was used to describe conversion to THA in the group of patients who initially underwent open reduction and internal fixation (ORIF). Debate continues about whether it is better to use a cemented or uncemented hemiarthroplasty to treat a displaced intracapsular fracture of the hip. The aim of this study was to attempt to resolve this issue for contemporary prostheses. A total of 400 patients with a displaced intracapsular fracture of the hip were randomized to receive either a cemented polished tapered stem hemiarthroplasty or an uncemented Furlong hydroxyapatite-coated hemiarthroplasty. Follow-up was conducted by a nurse blinded to the implant at set intervals for up to one year from surgery.Aims
Methods
The aim of this study was to develop a psychometrically sound measure of recovery for use in patients who have suffered an open tibial fracture. An initial pool of 109 items was generated from previous qualitative data relating to recovery following an open tibial fracture. These items were field tested in a cohort of patients recovering from an open tibial fracture. They were asked to comment on the content of the items and structure of the scale. Reduction in the number of items led to a refined scale tested in a larger cohort of patients. Principal components analysis permitted further reduction and the development of a definitive scale. Internal consistency, test-retest reliability, and responsiveness were assessed for the retained items.Aims
Methods
Outcome measures quantifying aspects of health in a precise,
efficient, and user-friendly manner are in demand. Computer adaptive
tests (CATs) may overcome the limitations of established fixed scales
and be more adept at measuring outcomes in trauma. The primary objective
of this review was to gain a comprehensive understanding of the
psychometric properties of CATs compared with fixed-length scales
in the assessment of outcome in patients who have suffered trauma
of the upper limb. Study designs, outcome measures and methodological
quality are defined, along with trends in investigation. A search of multiple electronic databases was undertaken on 1
January 2017 with terms related to “CATs”, “orthopaedics”, “trauma”,
and “anatomical regions”. Studies involving adults suffering trauma
to the upper limb, and undergoing any intervention, were eligible.
Those involving the measurement of outcome with any CATs were included.
Identification, screening, and eligibility were undertaken, followed
by the extraction of data and quality assessment using the Consensus-Based
Standards for the Selection of Health Measurement Instruments (COSMIN) criteria.
The review is reported according to the Preferred Reporting Items
for Systematic Reviews and Meta-Analyses (PRISMA) criteria and reg istered (PROSPERO: CRD42016053886).Aims
Materials and Methods
We performed a systematic review of the literature
pertaining to the functional outcomes of the surgical management
of acetabular fractures. A total of 69 articles met our inclusion
criteria, revealing that eight generic outcome instruments were
used, along with five specific instruments. The majority of studies
reported outcomes using a version of the d’Aubigne and Postel score,
which has not been validated for use in acetabular fracture. Few validated
outcome measures were reported. No psychometric testing of outcome
instruments was performed. The current assessment of outcomes in
surgery for acetabular fractures lacks scientific rigour, and does
not give reliable outcome data for either scientific comparison
or patient counselling. Take home message: The use of non-validated functional outcome
measures is a major limitation of the current literature pertaining
to surgical management of acetabular fractures; future studies should
use validated outcome measures to ensure the legitimacy of the reported

This study identifies early risk factors for symptomatic nonunion
of displaced midshaft fractures of the clavicle that aid identification

of fixation of displaced fractures of the radial neck. The 58 patients
with a mean age of 38.5 years (18 to 56), were treated in a non
randomised study with screws (n = 29) or a plate and screws (n =

evaluations were carried out up to one year post-operatively, using
the Broberg and Morrey functional evaluation score, range of movement,

This study describes the epidemiology and outcome
of 637 proximal humeral fractures in 629 elderly (≥ 65 years old) patients.
Most were either minimally displaced (n = 278, 44%) or two-part
fractures (n = 250, 39%) that predominantly occurred in women (n
= 525, 82%) after a simple fall (n = 604, 95%), who lived independently
in their own home (n = 560, 88%), and one in ten sustained a concomitant
fracture (n = 76, 11.9%). The rate of mortality at one year was
10%, with the only independent predictor of survival being whether
the patient lived in their own home (p = 0.025). Many factors associated
with the patient’s social independence significantly influenced
the age and gender adjusted Constant score one year after the fracture.
More than a quarter of the patients had a poor functional outcome,
with those patients not living in their own home (p = 0.04), participating
in recreational activities (p = 0.01), able to perform their own
shopping (p <
0.001), or able to dress themselves (p = 0.02)
being at a significantly increased risk of a poor outcome, which
was independent of the severity of the fracture (p = 0.001). A poor functional outcome after a proximal humeral fracture is
not independently influenced by age in the elderly, and factors

Our aim was to compare polylevolactic acid screws
with titanium screws when used for fixation of the distal tibiofibular
syndesmosis at mid-term follow-up. A total of 168 patients, with
a mean age of 38.5 years (18 to 72) who were randomly allocated
to receive either polylevolactic acid (n = 86) or metallic (n =
82) screws were included. The Baird scoring system was used to assess
the overall satisfaction and functional recovery post-operatively.
The demographic details and characteristics of the injury were similar
in the two groups. The mean follow-up was 55.8 months (48 to 66).
The Baird scores were similar in the two groups at the final follow-up.
Patients in the polylevolactic acid group had a greater mean dorsiflexion
(p = 0.011) and plantar-flexion of the injured ankles (p <
0.001).
In the same group, 18 patients had a mild and eight patients had
a moderate foreign body reaction. In the metallic groups eight had
mild and none had a moderate foreign body reaction (p <
0.001).
In total, three patients in the polylevolactic acid group and none
in the metallic group had heterotopic ossification (p = 0.246). We conclude that both screws provide adequate fixation and functional
recovery, but polylevolactic acid screws are associated with a higher

The increasing prevalence of osteoporosis in
an ageing population has contributed to older patients becoming
the fastest-growing group presenting with acetabular fractures.
We performed a systematic review of the literature involving a number
of databases to identify studies that included the treatment outcome
of acetabular fractures in patients aged >
55 years. An initial
search identified 61 studies; after exclusion by two independent
reviewers, 15 studies were considered to meet the inclusion criteria.
All were case series. The mean Coleman score for methodological
quality assessment was 37 (25 to 49). There were 415 fractures in
414 patients. Pooled analysis revealed a mean age of 71.8 years
(55 to 96) and a mean follow-up of 47.3 months (1 to 210). In seven
studies the results of open reduction and internal fixation (ORIF)
were presented: this was combined with simultaneous hip replacement
(THR) in four, and one study had a mixture of these strategies.
The results of percutaneous fixation were presented in two studies,
and a single study revealed the results of non-operative treatment. With fixation of the fracture, the overall mean rate of conversion
to THR was 23.1% (0% to 45.5%). The mean rate of non-fatal complications
was 39.8% (0% to 64%), and the mean mortality rate was 19.1% (5%
to 50%) at a mean of 64 months (95% confidence interval 59.4 to
68.6; range 12 to 143). Further data dealing with the classification
of the fracture, the surgical approach used, operative time, blood
loss, functional and radiological outcomes were also analysed. This study highlights that, of the many forms of treatment available
for this group of patients, there is a trend to higher complication
rates and the need for further surgery compared with the results
of the treatment of acetabular fractures in younger patients. Cite this article:
