To evaluate the perioperative complications associated with total en bloc spondylectomy (TES) in patients with spinal tumours, based on the extent and level of tumour resection. In total, 307 patients who underwent TES in a single centre were reviewed retrospectively. There were 164 male and 143 female patients with a mean age at the time of surgery of 52.9 years (SD 13.3). A total of 225 patients were operated on for spinal metastases, 34 for a malignant primary tumour, 41 for an aggressive benign tumour, and seven with a primary of unknown origin. The main lesion was located in the thoracic spine in 213, and in the lumbar spine in 94 patients. There were 97 patients who underwent TES for more than two consecutive vertebrae.Aims

Total knee arthroplasty (TKA) is one of the most successful surgeries to relieve pain and dysfunction caused by severe arthritis. However, it is a still big problem that there is a possibility of death in pulmonary embolism (PE) after TKA. We previously reported that there was more incidence of asymptomatic PE than estimated in general by detail examinations. But it was difficult to whom we decided to perform additional examinations except the patients with some doubtful symptoms. Therefore, we investigated detail of PE patients after primary TKA to find out anything key point in PE. Consecutive ninety-nine patients who underwent primary TKA from January 2015 to March 2018 were applied. There were 23 male and 76 females included, and the mean age was 73.7 years old. There were 96 cases of osteoarthritis, 2 cases of osteonecrosis and one of rheumatoid arthritis. A single knee surgery team performed all operations with cemented type prostheses and air tourniquet during operation. There were 35 cases of one-staged bilateral TKA and 64 of unilateral TKA. Detail examinations with contrast enhanced CT (CE-CT) and venous ultrasonography (US) were performed at the 3rd day after surgery. Next, we applied ultra sound cardiogram (UCG) to the patients diagnosed as PE by CE-CT, we checked right ventricular overload (RVO) to treat PE. These images were read by a single senior radiologist team.Introduction

Total Of the 54 patients who underwent TES for a primary tumour between
1993 and 2010, 19 died and four were lost to follow-up. In January
2012, a questionnaire was sent to the 31 surviving patients. This
included the short form-36 to assess HRQoL and questions about the
current condition of their disease, activities of daily living (ADL)
and surgery. The response rate was high at 83.9% (26/31 patients).
We found that most patients were satisfied and maintained good performance
of their ADLs. The mental health status and social roles of the HRQoL scores
were nearly equivalent to those of healthy individuals, regardless
of the time since surgery. There was significant impairment of physical
health in the early post-operative years, but this usually returned

The treatment of osteonecrosis of the femoral head (ONFH) in young and active patients remains a challenge. The purpose of this study was to determine and compare the clinical and radiographic results of the two different hip resurfacing systems; hemi-resurfacing and metal-on-metal total hip resurfacing in patients with ONFH. This study was a retrospective review of 20 patients with 30 hips who had ONFH and underwent hemi-resurfacing or total hip resurfacing between November 2002 and February 2006. We mainly performed hemi-resurfacing for early stage ONFH, and total hip resurfacing for advanced stages. Fifteen hips in 11 patients had a hemi-resurfacing component (Conserve, Wright Medical Co) with a mean age at operation of 50 years and an average follow-up of 5.5 years. Fifteen hips in 10 patients had a metal-on-metal total hip resurfacing component (Birmingham hip resurfacing, Smith & Nephew Co.) with a mean age at operation of 40 years and an average follow-up years.Introduction

Hip resurfacing has become a popular procedure for young active patients with osteonecrosis of the femoral head. However, it is not yet clear exactly how much osteonecrosis would permit this procedure and how much would be a contraindication. The aim of the present study was to analyze the resurfaced femoral head using finite element models and, in particular, to examine the influence of the extent of osteonecrosis and metaphysical stem shaft angles within the femoral head. We evaluated biomechanical changes at various extents of necrosis and implant alignments, using the finite element analysis method. We established three patterns of necrosis by depth from the surface of femoral head and five stem angles. Extension of necrosis as a quarter of femoral head diameter is type A, from a half is type B, and three-fourths is type C. We set five types of different stem angles from 125 to 145 degrees for the axis of femoral shaft. For these models, we evaluated biomechanical changes associated with the extent of necrosis and the stem alignment.Introduction

The position of the acetabular component affects the result of total hip arthroplasty(THA) in terms of postoperative dislocation, impingement, wear etc. However, as it is much difficult to place the component in the appropriate position for the cases of severe acetabular deformity, we used a Computed tomography(CT)-based navigation for THA in such cases. Therefore, the purpose of this study was to estimate the accuracy of a CT-based navigation in terms of acetabular component positioning in THA for severe acetabular deformities. 13 patients (1 man, 12 women), 14 hips underwent THA using a posterolateral approach with a CT-based navigation. The diagnoses were severe developmental dysplasia (Crowe group III, IV) in 6, ankylosis in 3, destructive arthritis after infection in 2, Charcot joint, and arthrodesed hip. And, we evaluated the differences of component position from the center of the anterior pelvic plane(APP), anteversion angle, and inclination angle relative to APP between the intraoperative data from the navigation system and the data from postoperative CT. Considering the intra-observer error, the measurement was done three times respectively and the mean value was accepted. We also estimated the difference between the component size planned and that implanted. The mean difference between intraoperative records and actual postoperative results of the component position shows 3.3 mm(range: 0–7.0, SD: 2.2) for the horizontal position, 3.2 mm(range: 0–9.7, SD: 4.5) for the vertical position, 4.4 mm(range: 2.0–7.7, SD: 1.6) for the antero-posterior position from the center of the APP, 1.3 degrees(range: 0–3.0, SD: 0.9) for the inclination and 2.9 degrees(range: 0.3–8.3, SD: 2.2) for the anteversion respectively. All components were placed in the safe zone by Lewinnek. The component size was predicted in 10/14(71.4%) hips. There were no complications related to the use of the navigation. This study showed the accuracy of cup positioning using a CT-based navigation in THA for the cases of severe acetabular deformity. We concluded that this system was a useful tool for surgeon to identify orientation, implant acetabular component at the precise position and angle, and to reduce the incidence of some complications especially for patients with these severe acetabular deformities.
Overall of all petients who underwent surgery, 71% had improved pain control, 53% regained mobility, 64% improved by at least one Frankel grade or maintained normal neurology and 39% regained normal urinary sphincter function. While 18% were bed bound preoperatively, only 5% were still in bed postoperatively. Perioperative mortality rate was 5.8% and morbidity was 21%. The median survival for the cohort was 352 days (11.7 months). The radical surgery group had a median survival of 438 days and the palliative group 112 days (P = 0.003).
We evaluated the possible induction of a systemic immune response to increase anti-tumour activity by the re-implantation of destructive tumour tissue treated by liquid nitrogen in a murine osteosarcoma (LM8) model. The tumours were randomised to treatment by excision alone or by cryotreatment after excision. Tissue from the tumour was frozen in liquid nitrogen, thawed in distilled water and then re-implanted in the same animal. In addition, some mice received an immunological response modifier of OK-432 after treatment. We measured the levels of interferon-gamma and interleukin-12 cytokines and the cytotoxicity activity of splenocytes against murine LM8 osteosarcoma cells. The number of lung and the size of abdominal metastases were also measured. Re-implantation of tumour tissue after cryotreatment activated immune responses and inhibited metastatic tumour growth. OK-432 synergistically enhanced the anti-tumour effect. Our results suggest that the treatment of malignant bone tumours by reconstruction using autografts containing tumours which have been treated by liquid nitrogen may be of clinical value.
We studied the safety of external fixation during post-operative chemotherapy in 28 patients who had undergone distraction osteogenesis (17, group A) or vascularised fibular grafting (11, group B) after resection of a tumour. Four cycles of multi-agent post-operative chemotherapy were administered over a mean period of 14 weeks (6 to 27). The mean duration of external fixation for all patients was 350 days (91 to 828). In total 204 wires and 240 half pins were used. During the period of post-operative chemotherapy, 14 patients (11 in group A, 3 in group B) developed wire- and pin-track infection. A total of ten wires (4.9%) and 11 half pins (4.6%) became infected. Seven of the ten infected wires were in periarticular locations. External fixation during post-operative chemotherapy was used safely and successfully for fixation of a vascularised fibular graft and distraction osteogenesis in 27 of 28 patients. Post-operative chemotherapy for malignant bone tumours did not adversely affect the ability to achieve union or cause hypertrophy of the vascularised fibular graft and had a minimal effect on distraction osteogenesis. Only one patient developed osteomyelitis which required further surgery.
The feasibility of bone transport with bone substitute and the factors which are essential for a successful bone transport are unknown. We studied six groups of 12 Japanese white rabbits. Groups A to D received cylindrical autologous bone segments and groups E and F hydroxyapatite prostheses. The periosteum was preserved in group A so that its segments had a blood supply, cells, proteins and scaffold. Group B had no blood supply. Group C had proteins and scaffold and group D had only scaffold. Group E received hydroxyapatite loaded with recombinant human bone morphogenetic protein-2 and group F had hydroxyapatite alone. Distraction osteogenesis occurred in groups A to C and E which had osteo-conductive transport segments loaded with osteo-inductive proteins. We conclude that scaffold and proteins are essential for successful bone transport, and that bone substitute can be used to regenerate bone.
We present a retrospective study of patients suffering from a variety of benign tumours in whom external fixators were used to treat deformity and limb-length discrepancy, and for the reconstruction of bone defects. A total of 43 limbs in 31 patients (12 male and 19 female) with a mean age of 14 years (2 to 54) were treated. The diagnosis was Ollier’s disease in 12 limbs, fibrous dysplasia in 11, osteochondroma in eight, giant cell tumour in five, osteofibrous dysplasia in five and non-ossifying fibroma in two. The lesions were treated in the tibia in 19 limbs, in the femur in 16, and in the forearm in eight. The Ilizarov frame was used in 25 limbs, the Taylor Spatial Frame in seven, the Orthofix fixator in six, the Monotube in four and the Heidelberg fixator in one. The mean follow-up was 72 months (22 to 221). The mean external fixation period was 168 days (71 to 352). The mean external fixation index was 42 days/cm (22.2 to 102.0) in the 22 patients who required limb lengthening. The mean correction angle for those with angular deformity was 23° (7° to 45°). At final follow-up all patients had returned to normal activities. Four patients required a second operation for recurrent deformity of further limb lengthening. Local recurrence occurred in one patient, requiring further surgery.
We describe a method of reconstruction using tumour-bearing autograft treated by liquid nitrogen in 28 patients. The operative technique consisted of The limb function was rated as excellent in 20 patients (71.4%), good in three (10.7%), fair in three (10.7%), and poor in two (7.1%). At the final follow-up six patients had died at a mean of 19.8 months after the operation, while 21 remained free from disease with a mean follow-up of 28.1 months (10 to 54). One patient is alive with disease. Bony union was seen at a mean of 6.7 months after the operation in 26 patients. Complications were encountered in seven patients, including three deep infections, two fractures, and two local recurrences. All were managed successfully. Our results suggest that this is a simple and effective method of biological reconstruction.
Although bone loss and ligamentous instability are usually indications for the use of constraining prostheses in revision total knee arthroplasty (TKA), several reports have documented a high rate of failure with these prostheses. We therefore tried using the cruciate retaining augmentable type prosthesis (NexGen CRA) if a good intraoperative ligamentous balance could be obtained with revision TKA. CRA was used on nine knees of seven patients with an average age of 71.7 years. The follow-up lasted for an average of 1.6 years. Clinical evaluation consisted of instability ratings, knee score, range of motion and %MA (mechanical axis) as an index of the alignment. These patients showed improvement varus instability from a mean of 7.0° preoperatively to a mean of 1.5° postoperatively, and in valgus instability from 5.3° to 1.3°. The knee score was significantly improved from 41 to 81, but the %MA resulted in an unsatisfactory improvement from −8.8% to 34.0%. The femoral anatomical-mechanical angle (FAMA) was measured as a parameter of bowing deformity of the femur, and the knees were divided into two groups, the bowing group consisting of the knees with an FMA of more than 8° (n=6, average 9.0°), the normal group of knees with an FAMA of less than 7° in FAMA (n=3, average 6.7°). The postoperative %MA showed a mean value of 40.9% for the normal group, and 30.6% in the bowing group. These results demonstrated that the discrepancy between FAMA for the bowing group(=9.0°) and the valgus angle of the stem of the femoral component (6.0°) was the cause of the malalignment in the bowing group. These clinical results suggest that the cruciate retaining augmentable type prosthesis can be used successfully for selected revision cases, but that malalignment in knees with bowed femora may remain a problem.
In 11 patients juxta-articular osteosarcoma around the knee was treated by intraepiphyseal excision of the tumour and reconstruction of the bone defect by distraction osteogenesis. Preoperative and postoperative chemotherapy was given to eight patients with high-grade tumours. The articular cartilage of the epiphysis and a maximum of healthy soft tissues were preserved. Distraction osteogenesis was then carried out. The mean gain in length was 9.7 cm. Full function of the limb was preserved in all except one patient, with a mean follow-up of 53.8 months. Treatment of juxta-articular osteosarcomas around the knee with joint preservation and biological reconstruction using distraction osteogenesis can give excellent functional results.
We describe a consecutive series of 26 patients with simple bone cysts who were treated by curettage, multiple drilling and continuous decompression by the insertion of either a cannulated screw or a pin. In the first 15 patients we used titanium cannulated screws (group 1) and in the next 11 a cannulated hydroxyapatite pin (group 2). Satisfactory healing was achieved in 12 patients in group 1 (80%) and in all in group 2. This technique seems to be a promising option for the treatment of simple bone cysts. The cannulated hydroxyapatite pin is recommended because of its higher success rate and the fact that it does not need to be removed.
We carried out limb lengthening in rabbits and then transplanted osteoblast-like cells derived from the tibial periosteum to the centres of distracted callus immediately after distraction had been terminated. Two weeks later the transaxial area ratio at the centre of the distracted callus and the bone mineral density (BMD) were significantly higher in the transplanted group, by 21% and 42%, respectively, than in the non-injected group or the group injected with physiological saline (p <
0.05). Callus BMD as a percentage of density in uninvolved bone was also significantly higher in the transplanted group (p <
0.05) than in the other two groups, by 27% and 20% in the second and fourth weeks, respectively (p <
0.05). Mechanically, the callus in the transplanted group tended to be stronger as shown by the three-point bending test although the difference in fracture strength was not statistically significant. Our results show that transplantation of osteoblast-like cells promotes maturity of the distracted callus as observed at the second and fourth weeks after lengthening. The method appears promising as a means of shortening the consolidation period of callus distraction and decreasing complications during limb lengthening with an external fixator.
We report the results of distraction osteogenesis (callotasis) for the reconstruction of extensive defects after the excision of skeletal tumours in the limbs. Bone transport was performed in ten patients (five osteosarcomas and five giant-cell tumours), shortening-distraction in three (two osteosarcomas and one Ewing’s sarcoma), and distraction osteogenesis combined with an intramedullary nail to reduce the time of external fixation in six (three osteosarcomas, two chondro-sarcomas, and one malignant fibrous histiocytoma). The mean length of the defects after excision of the lesion was 8.4 cm. The mean external fixation index was 39.5 days/cm for the group treated by bone transport, 34.1 days/cm for the shortening-distraction group, and 24.0 days/cm for the group treated by distraction and an intramedullary nail. Functional evaluation gave excellent results in 12 patients, good in five and fair in two. There were ten complications in 19 patients, all of which were successfully treated. We also classified reconstruction using distraction osteogenesis into five types based on the location of the defects after resection of the tumour: type 1, diaphyseal; type 2, metaphyseal; type 3, epiphyseal; type 4, subarticular reconstruction; and type 5, arthrodesis. Our results suggest that reconstruction using distraction osteogenesis provides bone which will develop sufficient biomechanical strength and durability. It is beneficial in patients with an expectation of long-term survival and in growing children.
