Over the past decade, there has been an increase in the number of total knee arthropalsty (TKA). Demand of TKA for the young patients who often have high physical demands is also increasing. However, the revision rate in such young patients is much higher due to polyethylene (PE) wear and instability (Julin J, Acta Orthop 2010). Therefore, next generation total knee prostheses are expected to decrease PE wear and to provide stability.

Although in vitro study such as wear simulator test provides important information about PE wear, we have often encountered the discrepancy between the in vitro results and in vivo results. Thus we have performed in vivo PE wear particle analysis, and showed that in vivo PE wear was affected by the design of articulating surface and the materials of femoral component and PE insert (Minoda Y, JBJS Am 2009). Medial pivot design, ceramic femoral component, and highly cross-linked PE decreased in vivo PE wear particle generation.

Patients who underwent bilateral staged TKAs were more likely to prefer medial pivot prosthesis or ACL-PCL retaining prosthesis than the other types of prostheses, because they feels “more stable overall” (Pritchett JW, J Arthroplasty 2011). In vivo fluoroscopic 3D analysis showed that medical pivot and bi-cruciate substituting designs restored physiological knee motion and provided higher reproducibility (Mueller J. Komistek RD, Trans ORS 2009, Iwakiri K, Trans ORS 2007).

The excellent mid-term clinical results of those newly introduced total knee prosthesis, such as alumina medial pivot TKA (Iida T, ORS 2008), medial pivot TKA (Mannan K, JBJS Br 2009, Kakachalions T, Knee 2009), ACL-PCL retaining TKA (Clouter JM, JBJS Am 1999), and highly cross-linked PE (Hodrick JT, CORR 2008), have been reported.

From the point of view of in vivo PE wear, in vivo stability, and the mid-term clinical results, we suspect that medial pivot prosthesis is one of the prostheses which meet the demand in future especially for young active patients.

Good mid-term results of Oxford UKA (OxUNI) for anteromedial osteoarthritis (OA) were reported. The designers of prosthesis reported a 98% 10-year survival rate for a combined series of phase I and II, and these findings were supported by published results from other series, with 10-year survival ranging from 91% to 98%. In order to obtain good results, the designers of this prosthesis mentioned the importance of adhering to strict indication for OxUNI, especially only for OA cases with intact anterior cruciate ligament (ACL). OxUNI combined with ACL reconstruction (ACLR) is a viable treatment option for only young active patients, in whom the ACL has been primarily ruptured. On the other hand, it was not clear whether the result of OxUNI combined with ACLR for OA with secondary ruptured ACL was good. In this study we compare the short-term results of OxUNI combined with ACLR for OA with secondary ruptured ACL with that for usual OA with intact ACL.

382 OxUNI were performed at two hospitals by one surgeon between January 2002 and August 2005. Among those, 367 cases, followed over two years postoperatively (272 patients, women: 283, men: 84) were assessed. Follow up ratio was 96.1%. The mean patient age at the time of surgery was 72.0 (47~93) years. The mean follow-up period was 39.3 (24~67) months. Thirty three knees of OA were treated with OxUNI combined with ACLR, by using synthetic graft. Clinical results were assessed by the Oxford Knee Score (OKS) and active range of motion (ROM). Patients are asked a series of 12 questions, and their response scores range from 0 (worse) to 4 (best) for each, yielding an overall score range of 0–48. All living patients were contacted, and the status of the implant was established at the time of last follow from hospital records. We evaluate the survival rate for OxUNI with or without ACLR, using the endpoint of revision for any reason.

The pre-and postoperative clinical scores were compared using the paired Student’s t-test. Survivor-ship curves were constructed using the Kaplan-Meier method, and survivorship between groups was compared using logrank and Wilcoxon methods. All analyses were performed using 95% confidence intervals and a P value of < 0.05 was considered significant.

The mean OKS at final follow-up was 42.1 (preoperative; 21.7), and the mean active ROM was 125.2° (preoperative; 113.4°). OKS and active ROM were significantly improved. There were no significant differences in OKS and active ROM between OxUNI with ACLR and OxUNI with intact ACL. Fourteen knees among 367 knees were revised; nine for loosening of tibial component, four for dislocation of bearing and one for progression of lateral OA. Overall 5-year survival rate was 95.6%. When survival rate was assessed separately with or without ACLR, that of OxUNI with intact ACL was 96.7% and that of OxUNI with ACLR was 83.8%. There was significant worse survival rate between the two groups (P=0.0071).

The 5-year survival rate for OxUNI with intact ACL was 96.7%, which was equivalent to those of original series from Oxford. However, 5-year survival rate for Oxford UKA with ACLR was 83.8% in our series. Four knees in nine of loosening of tibial component were replaced by OxUNI combined with ACLR. Therefore, even if ACL was reconstructed, the results of OxUNI for OA with secondary ruptured ACL was proved to be pessimistic.

There was significantly worse survival rate for OxUNI with ACLR, compared with OxUNI with intact ACL. So we conclude that combined ACL reconstruction and OxUNI for anteromedial OA with secondary ruptured ACL is not recommended, which must be treated with TKA.

Two collagen type IX gene polymorphisms that introduce a tryptophan residue into the protein’s triple-helical domain have been linked to an increased risk of lumbar disc disease. To determine whether a particular subset of symptomatic lumbar disease is specifically associated with these polymorphisms, we performed a prospective case-control study of 107 patients who underwent surgery of the lumbar spine. Patients were assigned to one of five clinical categories (fracture, disc degeneration, disc herniation, spinal stenosis without spondylolisthesis and spinal stenosis with spondylolisthesis) based on history, imaging results, and findings during surgery. Of the 11 tryptophan-positive patients, eight had spinal stenosis with spondylolisthesis and three had disc herniation. The presence of the tryptophan allele was significantly associated with African-American or Asian designation for race (odds ratio 4.61, 95% CI 0.63 to 25.35) and with the diagnosis of spinal stenosis with spondylolisthesis (odds ratio 6.81, 95% CI 1.47 to 41.95).

Our findings indicate that tryptophan polymorphisms predispose carriers to the development of symptomatic spinal stenosis associated with spondylolisthesis which requires surgery.

Introduction: Instability of the anterior horn of the medial meniscus (MM) has been described as dislocating, subluxating or hypermobile, but it is still controversial whether segments of the MM of the knee were surgically treated by arthroscopic stabilization. The average age of the patients was 28.7 (range 12 to 56). There were 9 men and 4 women. All patients complained of medial knee pain and felt tenderness in the medial joint space, most of them on the anterior side. None showed an apparent tear of the meniscus by arthroscopy or on MRI images, but all arthroscopically showed hypermobility (or easy dislocation from the edge of the tibial plateau) of the anterior to middle segment of the MM. No other apparent pathological changes were found. Six knees had marked limitations in the range of knee motion before operation. Arthroscopic stabilization of the hypermobility was performed in order to restrain the movement of the MM by fixing it to the tibial edge, using staples (2 cases), Kirschner wires (2 cases) or suture anchors (9 cases). Using the Japanese Orthopaedic Association meniscus injury score (maximum 100 points), the result was evaluated. The average follow up period was 20.1 months (range 9 to 49 months)

Results: The result of arthroscopic fixation was satisfactory (excellent in 8 cases good in 1, fair in 1, and poor in 1). The average meniscus score at follow up was 87.8, while that of before operation was 41.9. It is suggested that instability of the anterior segment of the MM can be effectively treated by arthroscopic fixation of this site.

Discussion: Since all of the knees in this study had an isolated lesion of instability in the anterior segment of the MM, the marked improvement in medial knee pain that resulted from fixation of this site does show that this lesion can be symptomatic. After excluding other possible pathological lesions, stabilization of this lesion by arthroscopic fixation is a good choice of treatment.

Recent studies of the fibroblast growth factor receptor 3 (FGFR3) gene have established that achondroplasia and hypochondroplasia are allelic disorders of different mutations.

To determine whether the genotype could be distinguished on the basis of the phenotype, we analysed height, arm span, and skeletal radiographs from 23 patients with achondroplasia and the G380R mutation of FGFR3 and eight with hypochondroplasia and the N540K mutation. Both conditions share the classical pathological features of micromelic short stature, reduced or unchanged interpedicular distances in the lumbar spine, disproportionately long fibulae, and squared and shortened pelvic ilia. These were significantly more severe in the G380R patients than in the N540K patients.

Our findings have shown a firm statistical correlation between the genotype and the phenotype, although there were a few exceptional cases in which there was phenotypic overlap between the two conditions.
