Summary. Optimum position of pedicle screws can be determined preoperatively by CT based planning. We conducted a comparative study in order to analyse manually determined pedicle screw plans and those that were obtained automatically by a computer software and found an agreement in plans between both methods, yet an increase in fastening strengths was observed for automatically obtained plans. Hypothesys. Automatic planning of pedicle screw positions and sizing is not inferior to manual planning. Design. Prospective comparative study. Introduction. Preoperative planning in spinal deformity surgery starts by a proper selection of implant anchors throughout the instrumented spine, where pedicle screws provide the optimum choice for bone fixation. In the case of severe spinal deformities, dysplastic pedicles can limit screw usage, and therefore studying the anatomy of vertebrae from preoperative images can aid in achieving the safest screw position through optimal fastening strength. The purpose of this study is to compare manually and automatically obtained preoperative pedicle screw plans. Materials and Methods. CT scans of 17 deformed thoracic spines were studied by two experienced spine deformity surgeons, who placed 316 pedicle screws in 3D using a software positioning tool by aiming for the safest trajectory that permitted the largest possible screw sizes. The resulting manually obtained screw sizes, trajectory angles, entry points and normalised fastening strengths were compared to those obtained automatically by a dedicated computer software that, basing on vertebral anatomy and bone density in 3D, determined optimal screw sizes and trajectories. Results. Statistically significant differences were observed between manually and automatically obtained plans for screw sizes (p < 0.05) and trajectory angles (p < 0.001). However, for automatically obtained plans, screws were not smaller in diameter (p < 0.05) or shorter in length (p < 0.001), while screw normalised fastening strengths were higher (p < 0.001). Conclusions. In comparison to manual planning, automatically obtained plans did not result in smaller screw diameters or shorter screw lengths, which is in agreement with the definition of the pull-out strength, but in different screw trajectory angles, which is reflected by higher normalised fastening strengths. In multilevel posterior cervical instrumented fusions, extending the fusion across the cervico-thoracic junction at T1 or T2 (CTJ) has been associated with decreased rate of re-operation and pseudarthrosis but with longer surgical time and increased blood loss. The impact on patient reported outcomes (PROs) remains unclear. The primary objective was to determine whether extending the fusion through the CTJ influenced PROs at 3 and 12 months after surgery. Secondary objectives were to compare the number of patients reaching the minimally clinically important difference (MCID) for the PROs and mJOA, operative time duration, intra-operative blood loss (IOBL), length of stay (LOS), discharge disposition, adverse events (AEs), re-operation within 12 months of the surgery, and patient satisfaction. This is a retrospective analysis of prospectively collected data from a multicenter observational cohort study of patients with degenerative cervical myelopathy. Patients who underwent a posterior instrumented fusion of 4 levels of greater (between C2-T2) between January 2015 and October 2020 with 12 months follow-up were included. PROS (NDI, EQ5D, SF-12 PCS and MCS, NRS arm and neck pain) and mJOA were compared using ANCOVA, adjusted for baseline differences. Patient demographics, comorbidities and surgical details were abstracted. Percentafe of patient reaching MCID for these outcomes was compared using chi-square test. Operative duration, IOBL, AEs, re-operation, discharge disposittion, LOS and satisfaction were compared using chi-square test for categorical variables and independent samples t-tests for continuous variables. A total of 206 patients were included in this study (105 patients not crossing the CTJ and 101 crossing the CTJ). Patients who underwent a construct extending through the CTJ were more likely to be female and had worse baseline EQ5D and NDI scores (p> 0.05). When adjusted for baseline difference, there was no statistically significant difference between the two groups for the PROs and mJOA at 3 and 12 months. Surgical duration was longer (p 0.05). Satisfaction with the surgery was high in both groups but significantly different at 12 months (80% versus 72%, p= 0.042 for the group not crossing the CTJ and the group crossing the CTJ, respectively). The percentage of patients reaching MCID for the NDI score was 55% in the non-crossing group versus 69% in the group extending through the CTJ (p= 0.06). Up to 12 months after the surgery, there was no statistically significant differences in PROs between posterior construct extended to or not extended to the upper thoracic spine. The adverse event profile did not differ significantly, but longer surgical time and blood loss were associated with construct extending across the CTJ

Aim:. Historically, anterior decompression followed by posterior fusion has been the surgical management of choice in spinal tuberculosis. Due to theatre time being at a premium, we have evolved to performing anterior only debridement, allograft strut reconstruction and instrumentation for tuberculosis in the adult thoracic spine. The aim of this study is to review the safety and the efficacy of this treatment. Methods:. Twenty-eight adult thoracic tuberculosis patients were identified where anterior only surgery had been performed. These were all in the mid-thoracic spine as circumferential surgery is still favoured in thoracolumbar disease. The surgery was performed by a single surgeon at a tertiary hospital. Following transthoracic aggressive debridement, allograft humeral shafts were cut to size and inserted under compression and the spines instrumented with the use of screw-rod constructs into the body above and below. A retrospective review of clinical notes and radiological studies was performed. Results:. Twenty-seven of the patients presented with altered neurology; 2 had only sensory changes while 25 presented with paraparesis; 22 of these patients were unable to walk. The average surgical time was 2 hours 20 minutes with a median blood loss of 726 ml. The majority of patients had 2 vertebral bodies involved and required an average of a 4 body fusion. Surgical complications included inadvertent opening of the diaphragm in 1 patient and 1 patient deteriorated neurologically post operatively. 21 of 28 patients recovered to independent mobility at their latest follow-up appointment. 1 patient showed no recovery, 3 had some motor recovery that was not useful, 1 had some sensory but no motor recovery. 16 of 28 patients have documented bony fusion with no evidence of instrumentation failure in any patients. Conclusion:. Anterior only debridement, allograft strut reconstruction and instrumented fusion for the treatment of thoracic spinal tuberculosis is a safe and effective alternative to circumferential surgery in the adult patient

The use of cervical pedicle screws as anchors in posterior reconstruction surgery has not been widely accepted due to the neurological or vascular injury. We thus sought to investigate the accuracy of free-handed pedicle screw placement in the cervical and upper thoracic spine at the early stage of clinical application. Eight patients (five males and three females) were included in this study. Mean age was 63 years (31 to 78 years). There were three patients with rheumatoid arthritis, three with cervical fracture-dislocation, and two with spinal metastasis. Twenty-four pedicle screws (3.5 mm diameter: Vertex, Medtronic Sofamordanek) were placed into the pedicle from C2 to T2 level by free-handed technique2). Grade of breaching of pedicle cortex was divided into four groups (Grade 0–3). In addition, screw axis angle (SAA) were calculated from the horizontal and sagittal CT images and compared with pedicle transverse angle (PTA). Furthermore, perioperative complications were also examined. Our free-handed pedicle screw placement with carving technique is as follows: A longitudinal gutter was created at the lamina-lateral mass junction and then transverse gutter perpendicular to the longitudinal gutter was made at the lateral notch of lateral mass. The entry point of the pedicle screw was on the midline of lateral mass. Medial pedicle cortex through the ventral lamina was identified using the probes to create the hole within the pedicle. The hole was tapped and the screw was gently introduced into the pedicle to ensure the sagittal trajectory using fluoroscopy. In the transverse direction, 22 out of 24 screws (92%) were entirely contained within the pedicle (Grade 0). In contrast, only teo screws (8%) produced breaches less than half the screw diameter (Grade 1). In the sagittal direction, all screws were within the pedicle (Grade 0). Screw trajectories were not consistent with anatomical pedicle axis angle; the mean SAA were smaller than the mean PTA at all levels. The pedicle diameter ranged from 3.9 to 9.2 mm. The mean value gradually increased toward the caudal level. There were no neurological and vascular complications related to screw placement

Adolescent idiopathic scoliosis is a three-dimensional deformity of the spine, affecting 1–3% of the population. Most cases are treated conservatively. Curves exceeding 45° in the thoracic spine and 40° in the lumbar spine may require correction and fusion surgery, to limit the progression of the curve and prevent restrictive pulmonary insufficiency (curves above 70°). When fusion is required, it may be performed either by posterior or anterior approaches. Posterior is useful for thoracic (Lenke I) curves, notably to correct the thoracic hypokyphosis frequently observed in AIS. Anterior approaches by thoraco-lombotomies allow an effective correction of thoraco-lumbar and lumbar curves (Lenke V and VI), with fewer levels fused than with posterior approaches. However, the approach requires diaphragm splitting and one may be concerned about the long-term pulmonary consequences. The literature provides conflicting insight regarding the consequences of the approach in anterior scoliosis correction, the interpretation of the results being difficult knowing that the correction of the scoliosis itself may improve pulmonary function. This is a retrospective observational study done at a Tertiary Institution. The HRQOL scores have been collected as a prospective cohort. Clinical and radiographic data was collected from patients charts and analysed by two senior surgeons. A cohort of 64 patients were operated in the given time period. 50 patients met the inclusion criteria. No major complications were reported. The Union rate was 100% and no post operative complications were noted. Pre and post SRS scores improved in all patients. The Anterior approach for Lenke V AIS gives great surgical exposure and allows for excellent correction of Cobb angle with minimal risk to the patient

Symptomatic lumbar spinal stenosis is a common entity and increasing in prevalence. Limited evidence is available regarding patient reported outcomes comparing primary vs revision surgery for those undergoing lumbar decompression, with or without fusion. Evidence available suggest a lower rate of improvement in the revision group. The aim of this study was to assess patient reported outcomes in patients undergoing revision decompression, with or without fusion, when compared to primary surgery. Patient data was collected from the Canadian Spine Outcomes Research Network (CSORN) database. Patients undergoing lumbar decompression without or without fusion were included. Patients under 18, undergoing discectomy, greater than two level decompressions, concomitant cervical or thoracic spine surgery were excluded. Demographic data, smoking status, narcotic use, number of comorbidities as well as individual comorbidities were included in our propensity scores. Patients undergoing primary vs revision decompression were matched in a four:one ratio according to their scores, whilst a separate matched cohort was created for those undergoing primary vs revision decompression and fusion. Continuous data was compared using a two-tailed t-test, whilst categorical variables were assessed using chi-square test. A total of 555 patients were included, with 444 primary patients matched to 111 revision surgery patients, of which 373 (67%) did not have fusion. Patients undergoing primary decompression with fusion compared to revision patients were more likely to answer yes to “feel better after surgery” (87.8% vs 73.8%, p=0.023), “undergo surgery again” (90.1% vs 76.2%, P=0.021) and “improvement in mental health” (47.7% vs 28.6%, p=0.03) at six months. There was no difference in either of these outcomes at 12 or 24 months. There was no difference between the groups ODI, EQ-5D, SF 12 scores at any time point. Patients undergoing primary vs revision decompression alone showed no difference in PROMs at any time point. In a matched cohort, there appears to be no difference in improvement in PROMS between patients undergoing primary vs revision decompression, with or without fusion, at two year follow-up. This would suggest similar outcomes can be obtained in revision cases

Aim. Empiric antibiotic therapy for suspected pyogenic spondylodiscitis (SD) should be initiated immediately with severely ill patients and may also be necessary for culture-negative SD. The aim of this study was to infer an appropriate empiric antibiotic regimen by analyzing the antimicrobial susceptibility of isolated pathogens from microbiologically proven pyogenic spondylodiscitis. Method. We performed a retrospective review of adult patients with clinically proven SD treated at our level 1 trauma center between 2013 and 2020. Demographic data, radiologic findings, and treatment modalities were evaluated. The appropriateness of empiric antibiotic regimens was assessed based on the antibiograms of the isolated pathogens. Anamneses were used to distinguish between community-acquired (CA) and healthcare-associated (HA) pathogens, which included cases that had a hospital stay or invasive intervention in the past 6 months. Results. A total of 155 patients (male: N=88; female: N=67; mean age 66.1 ± 12.4 years) with SD were identified. In n= 74 (47.7%) cases, the infections were associated with the healthcare system (HA). N=34 (21.9%) patients suffered from sepsis. The lumbar spine was involved in 47.1% of the cases, the thoracic spine in 37.3%, and the cervical spine in 7.8%. In 7.8% of the cases, SD occurred in multiple spinal segments. N=96 (62.0%) patients were treated surgically. The mean hospital stay was 36.4 ± 36.3 days. Antibiograms of n=45 patients (HA: N=22; CA: N=23) could be retrospectively evaluated: The most frequently identified pathogens were Staphylococcus aureus (46.7%), Coagulase-negative Staphylococci (17.8%), Enterobacteriaceae (15.6%) and Streptococcus species (15.6%). Overall, 82.2% (HA: 68.2%; CA: 95.5%) of the isolated pathogens were sensitive to piperacillin/tazobactam, 77.8% (HA: 81.8%; CA: 72.2%) to vancomycin, 64.4% (HA: 68.2%; CA: 59.1%) to clindamycin, and 55.6% (HA: 36.4%; CA: 72.7%) to ceftriaxone. To a combination of vancomycin plus meropenem 97.8% of pathogens were sensitive (HA: 95.5%; CA: 100.0%), to vancomycin plus ciprofloxacin 91.1% (HA: 86.4%; CA: 95.7%), and to vancomycin plus cefotaxime 93.3% (HA: 90.9%; CA: 95.7%). In 14 cases, empiric antibiosis was adjusted based on the results of the antibiogram. Conclusions. Antibiotic resistance of CA SD pathogens differed significantly from HA SD. The identification of the pathogen and the analysis of its susceptibility guides the antibiotic therapy. Vancomycin in combination with a carbapenem, broad-spectrum cephalosporin, or fluoroquinolone may be appropriate for empiric treatment of HA SD

The evolution of operative technology has allowed correction of complex spinal deformities. Neurological deficits following spinal instrumentation is a devastating complication and the risk is especially high in those with complex sagittal and coronal plane deformities. Prior to intraoperative evoked potential monitoring, spinal cord function was tested using the Stagnara Wake up test, typically performed after instrumentation once the desired correction has been achieved. This test is limited as it does not reflect the timeframe in which the problem occurred and it may be dangerous to some patients. Intraoperative neuromonitoring allows timely feedback of the effect of instrumentation and curve correction on the spinal cord. Pedicle screws that are malpositioned can result in poor fixation or neuronal injury. Evoked EMG monitoring can aid in accurate placement. A positive EMG response can alert the surgeon to a potential pedicle breech and allow them to reassess the placement of their hardware intraoperatively. The stimulation threshold is affected by the amount of surrounding bone acting as an insulator to electrical conduction and is variable in different regions of the spine. In the non-deformed, lumbar spine stimulation thresholds have been established. Such guidelines have not been well-developed for the thoracic spine, or for severely scoliotic spines. Thus our primary objective was to compare the stimulation threshold of the apical pedicle on the concave side to the stimulation threshold of the pedicles at the upper and lower instrumented levels. Intraoperative EMG stimulation thresholds were done at 192 apical pedicles on the concave side of the deformity and then compared to those thresholds found at 169 terminal level pedicles. Only pedicles for which a stimulation threshold was found were reported and excluded those where a breech was suspected. The lowest stimulation required for an EMG response was documented to a maximum stimulation of 20 mA. The mean threshold at the apex was 16.62 milliamps (mA) compared to 18.25mA at the terminal levels. This was compared with the t-test and showed a statistically significant difference (p<0.05). In this study we report only the thresholds for the concave side, the pedicle that is most likely to be reduced in size. The threshold for stimulation is reduced compared to those seen at the highest and lowest instrumented level. Most of the apexes are located in the mid-thoracic spine with the highest instrumented levels being in the high thoracic spine and the lowest levels being in the lumbar spine. This study provides preliminary evidence that the apical, concave pedicle has a lower threshold than the end pedicles and one cannot rely on established thresholds from different areas of the spine. The surgeon should be cognisant of these differences when instrumenting at the apical level. Ongoing work is examining the convex apex threshold as well as the relationship between the effect of age and a diagnosis other than adolescent idiopathic scoliosis

This study examines the case of a spondylodiscitis in the thoracic spine caused by MRSA which led to two orthopaedic surgeries followed by rehabilitation. A 72.6 year old woman suffered a cutaneous infection with herpes zoster on the right dorsal thorax – 2 weeks later she presented a sepsis by MSSA. 2 month later she lamented sever pain in the thoracic column. She were hospitalized presenting a sepsis by MRSA. One month later it was found an infectious spondylodiscitis from thoracic vertebra T 8 to T 11 with destruction of the intervertebral spaces. To eliminate the infection and stabilize the dorsal column she was undertaken an first orthopaedic surgery by dorsal decompression and dorsal spondylodesis from T 6 – L 2; intraoperative microbiology: MRSA. 3 month later she was undertaken a second surgery by a lateral transthoracic decompression and intervertebral stabilization from T 9 – T 10 with tricortical bone chips and inlay of sponge with Calcibon and Gentamycin. The following rehabilitation took her to a reasonable result. The cost of the first treatment with dorsal stabilization was € 17.694,24, the second surgery was € 13.678,88; the cost of both rehabilitations was € 4.160,00. The finally costs for the whole treatment for the insurance was € 47,442,62. This retrospective case report shows the high costs for a treatment of spondylodiscitis caused by MRSA, not taking in consideration the harm and prolonged pain of the patient

Aim. To evaluate efficacy of a one stage posterior approach in decompression and eradication of infection in TB spine. Background. The classic operation for TB spine is anterior spine debridement. This involves a trans-thoracic, or retroperitoneal approach, thus increasing morbidity in an already compromised patient. The anterior procedure in the form of the Hong Kong operation is aimed at decompressing the spine, and debridement of necrotic tissue. If kyphosis is a major problem, its correction requires a posterior procedure, often not at the same sitting. Material and Method. A retrospective review of patients treated surgically for TB Spine during the time period 2009–2012. We examined the records of those patients that were treated by a posterior only approach. We took note of the demographics of the patients. We measured the efficacy of the decompression by measuring the pre op and post op neurologic status as measured by the Frankel grading. The efficacy of debridemide was assessed by measuring the preoperative and follow up ESR. Results. We identified 11 patients for review, 8 male and 3 females. 8 were HIV positive. The disease affected the thoracic spine. The average follow up was 12 months. There was good correction of the deformity and this was maintained throughout the follow up period. The ESR decreased in all the patients. Neurologic improvement was noted in 5 patients and no patients deteriorated. Statistical methods to quantify these changes were not significant because of the small numbers. Conclusion. In our environment a number of patients are immunocompromised by the HIV virus. A trans thoracic approach increases the morbidity in these patients. Effective decompression and debridement can be achieved by the posterior only approach. NO DISCLOSURES

Introduction and aims. Solitary fibrous tumours (SFT) are rare soft tissue sarcomas. Challenges in management include the variation in anatomical location and uncertain malignant potential. We retrospectively reviewed our experience with the aim of formulating guidelines on appropriate treatment. Methods. An electronic database identified patients with SFT presenting between 2003–2011. Clinical records were reviewed. Results. 23 patients were identified. Mean age was 50.6 years (12–77 years). The anatomic location was lower limb in 8, upper limb 4, intrathoracic 3, retroperitoneal 3, buttock and perianal regions 2, pelvis 1, thoracic spine 1 and brain 1. 10 cases were histologically benign, 5 had malignant potential and 8 were malignant. In 12 cases there was a complete surgical resection. In 6 cases there was a positive margin or piecemeal excision. There were 4 local recurrences and 5 patients with metastases. Completeness of record keeping varied with times and surgical speciality. Conclusions. SFT occur infrequently, are difficult to diagnose, have an unpredictable course and present to a variety of surgical specialists. In order to improve outcomes, we recommend surgical and histological margins are accurately recorded and audited whatever surgical specialty leads the surgery, ideally within a sarcoma MDT. Complete resection at first attempt by specialists with an understanding of the need for complete resection is the goal

Aim. The purpose of this study was to establish the diagnostic utility and spectrum of fluoroscopy guided percutaneous transpedicular biopsies of the thoraco-lumbar spine performed at our institution and to review some aspects regarding the diagnosis of spinal tuberculosis (TB). Methods. A retrospective study was performed on a consecutive series of 48 patients who underwent fluoroscopy guided percutaneous transpedicular biopsies of the spine performed at our institution. Biopsy specimens were sent for microscopy, culture and sensitivity (MC&S) as well as for TB microscopy, culture and polymerase chain reaction (PCR) and for histology. Results. There were 26 females and 22 males. The ages ranged from 23 to 79 years with a mean of 47 years. Fifteen biopsies were performed in the thoracic spine and 33 in the lumbar spine. A diagnosis was established in 83% of patients. The most common diagnosis, made in 58% of cases, was TB. The second most common diagnosis was metastatic disease, diagnosed in 15%. In 3 patients a diagnosis was made of concomitant TB and metastatic adenocarcinoma. No complications were encountered. Regarding the diagnosis of spinal TB we found that microscopy for alcohol and acid fast bacilli has a very low yield, with TB cultures, PCR and histology delivering a higher yield. TB PCR was also found to be positive in some cases where the TB cultures were negative which had implications for our institution's protocol regarding performance of TB PCR testing. Conclusion. Fluoroscopy guided percutaneous transpedicular biopsies of the thoraco-lumbar spine is a safe and effective tool in the diagnostic work-up of a patient with a spinal lesion of uncertain cause. We recommend that in all cases specimens should be send for histology and bacteriological studies and if spinal TB is suspected samples should be sent for histology, MC&S and PCR. NO DISCLOSURES

Introduction:. Trauma is endemic in South Africa. The upper thoracic spine is extremely difficult to image and assess clearly with frontline x-rays resulting in up to 22% of proximal fractures being missed. Aim:. To review a series of patients with proximal thoracic fractures. Methods:. Thirty-three patients with proximal thoracic fractures in the T1–T4 area managed in a spinal unit were identified. A retrospective review of medical records and radiology was undertaken. Demographic data, mechanism of injury, diagnostic modalities, level and type of fracture, neurological status, associated injuries, hospital stay, management, complications and outcome was recorded. Results:. There were 21 males and 12 females, with a median age of 31.8 years. Aetiology was 21 MVA passengers, 8 drivers, 1 pedestrian, 1 assault, 1 bicycle and 1 hanglider accident. Delay in diagnosis was 1 day in 8, 2–5 days in 2 and greater than 2 weeks in 5. The fractures were A1 in 7, A3 in 14, B1 in 7, C1 in 2 and C2 in 2. Twenty three patients had neurological compromise, 13 being complete. Twenty-three had associated chest and head injuries. Hospital stay was a mean of 27 days (maximum 246) and ICU stay median 14 (maximum 115) days. Twenty-six patients underwent surgery, posterior instrumented fusion being the commonest procedure. Although the surgery did not change the median kyphosis (25° preop to 20°at 1 year and 21°at 2 years), the most kyphotic patients were improved (55°to 45°). Conclusion:. A high index of suspicion for proximal thoracic fractures needs to be maintained in high energy injuries, especially MVA passengers, where there is chest injury. Prompt exclusion by appropriate special investigations is mandatory. Once recognized, they can be adequately managed with posterior instrumented fusion, although these patients are resource intensive due to the associated neurological impairment and chest injuries

We prospectively studied the use of intercostal EMG monitoring as an indicator of the accuracy of the placement of pedicle screws in the thoracic spine. We investigated 95 thoracic pedicles in 17 patients. Before insertion of the screw, the surgeon recorded his assessment of the integrity of the pedicle track. We then stimulated the track using a K-wire pedicle probe connected to a constant current stimulator. A compound muscle action potential (CMAP) was recorded from the appropriate intercostal or abdominal muscles. Postoperative CT was performed to establish the position of the screw. The stimulus intensity required to evoke a muscle response was correlated with the position of the screw on the CT scan. There were eight unrecognised breaches of the pedicle. Using 7.0 mA as a threshold, the sensitivity of EMG was 0.50 in detecting a breached pedicle and the specificity was 0.83. Thoracic pedicle screws were accurately placed in more than 90% of patients. EMG monitoring did not significantly improve the reliability of placement of the screw

Introduction. The resurgence of TB worldwide has several underlying causes, but HIV infection has undoubtedly been a key factor in the current TB epidemic. Since TB is endemic in the developing world the influence of HIV is of concern, particularly with the emergence of multi-drug-resistant strains. The remarkable susceptibility of patients with AIDS to develop TB has shown the critical role of CD4 lymphocytes in protective immunity. In the absence of immunological surveillance by CD4 cells, 5-10% of persons with latent foci of TB reactivate each year. Aim. This paper highlights the presentation and outcome following treatment in HIV patients with spinal TB. Methods. 81 HIV+ve patients with spinal tuberculosis were prospectively evaluated between 2006 and 2007. The mean age was 31 years and 63% were females. The thoracic spine was affected in 45, lumbar (33) and cervical (3). Non-contiguous lesions were noted in six patients. Sixty-six (81%) patients had neurological deficit. The mean Hb was (10.1gm/dl), mean WCC 4.9, mean lymphocyte count was 1.8, mean ESR 79mm/h and the mean CD4 count was 268 cell/cumm. Co-morbidities were seen in 68% of patients. All patients were optimised prior to treatment. Posterolateral decompression was performed in 29 cases, anterior decompression (25), needle biopsy (13), incision and drainage 5 and 9 were treated non-operatively. Medication included ARV (72 patients) and anti-TB in all patients. Results. The mean follow-up was 21 months. Eleven (13.5%) patients developed wound infection and one child died. Complete recovery occurred in 23 patients (35%). The mean CD4 count was 341 cell/cumm. Conclusion. The short term results are encouraging. These patients are best managed by a multidisciplinary team to monitor potential complications from dual therapy, to ensure compliance and adequate nutrition

The optimal management of patients with the diagnosis of a spinal epidural abscess (SEA) remains controversial. The purpose of this study was to describe the clinical characteristics of patients presenting with spontaneous SEA and to correlate presentation and treatment with clinical and neurological outcome. A retrospective review of the medical records and radiology of patients with a diagnosis of SEA, treated between September 2003 and December 2010, at a tertiary referral hospital was performed. A total of 46 patients were identified including 27 males and 19 females. Mean age was 61 years (range, 30 – 86 years). At presentation, all patients had axial pain and 67% had a neurological deficit, out of which one third had paraplegia or quadriplegia. 32% patients were febrile. Diabetes was the most common risk factor (30%) followed by malignancy (17%), intravenous drug use (6%) and alcoholism (2%). Organisms were cultured in 44 patients with Methicillin Sensitive Staphylococcus Aureus most common (68%), followed by Methicillin Resistant Staphylococcus Aureus (14%). The epidural abscess was located in the lumbar spine in 24 patients, thoracic spine in 11 patients and cervical spine in 11 patients. 61% of patients had a concurrent source of septic focus on presentation, including psoas abscess (24%), facet joint septic arthritis (15%), pneumonia (11%), infective endocarditis (7%) and urosepsis (4%). 26% of patients were treated non-operatively, with computed tomography-guided aspiration and/or intravenous antibiotics based on cultures, whereas 74% underwent surgical decompression with or without fusion in combination with antibiotics. The mean inpatient hospital stay was 42 days (range, 2 – 742 days) and 34% of patients required an average of 40 days of Intensive Care Unit admission. At time discharge from hospital, of the patients managed nonoperatively, 33% had improved neurological function, 17% had worsened neurological function, 17% died and data was unavailable in 33%. Of the patients treated with surgery, 74% had improved neurological function, 6% remained unchanged, 6% had worsened neurologic function, 6% died and data was unavailable in 9% at time of discharge. SEA remains a severe condition associated with multiple septic foci and significant morbidity. Surgical decompression combined with antibiotics is associated with superior neurologic recovery compared with non-operative management, however a significant proportion of patients still deteriorate or die. Early diagnosis and management may prevent or reduce permanent neurologic deficit

Aim. Spondylodiscitis and vertebral osteomyelitis can lead to long-term sequelae if not diagnosed and treated promptly and appropriately. The Royal National Orthopaedic Hospital (RNOH) has devised a new spinal infection referral system within the UK that allows cases to be discussed in a specialist multi-disciplinary (MDT) forum. National guidelines were devised in 2013 to help guide treatment, which recommends both tissue biopsies from the affected region and a MRI of the entire spine. The aims of this study were to assess the current treatment and referral practices and compare them with the set guidelines. It is hypothesised that a high percentage of patients are started on antibiotics without a biopsy or a positive set of blood cultures, a low percentage of patients are referred without undergoing a MRI of the full spine and that there is a long delay in referral to the MDT. Method. A retrospective case study analysis was carried out on all spinal infection referrals received by the Royal National Orthopaedic Hospital over a 2-year period (2014–16), using the standards set by the current national guidelines. Clinical features, haematology results, imaging, biopsy results, treatment and outcome were all reviewed. Three key areas were addressed; whether antibiotics were commenced before positive cultures or biopsy, whether a MRI of the entire spine was performed and the time taken for referral from the onset of symptoms. Results. A total of 49 cases were identified, the average age was 56 years and the majority were male (67%). The most common organisms grown were staphylococcus aureus and mycobacterium tuberculosis. Lumbar (37%) and thoracic (31%) spine were the most predominant regions affected. As predicted only 42% of the patients were referred with a whole spine MRI, only 33% had a biopsy and 29% had positive blood cultures before starting antibiotics. The average time to referral was 62 days overall (10 days for acute referrals). Conclusions. This multicentre study highlights a number of key concerns with both the referral and treatment of spinal infections. New national infection guidelines recommend a tissue sample prior to antibiotics; however this study showed that only 45% of patients had either a biopsy or positive blood culture before starting antibiotics. The time to referral was prolonged and the majority of patients did not receive a whole spine MRI. Overall this confirms the study's hypothesis and the need for better management of this complex and debilitating condition
