Background. Sciatic nerve palsy is a relatively rare, but one of the serious complications after total hip arthroplasty (THA). The prevalence of nerve palsy after THA has been reported to range from 0.3% to 3.7%. Previous authors have speculated that causes could include overlengthening, compression from a hematoma, from extruded metylmethacrylate, or from retractor placement, or laceration from a screw used in the acetabular component. Leg lengthening more than 4 cm was associated with sciatic nerve palsy in the past literature. But there is no report about maximum safety leg lengthening to prevent sciatic nerve palsy significantly. The purpose of this study was to identify the safety rage of leg lengthening to prevent sciatic nerve palsy in THA for the patients with adult hip dislocation. Methods. Forty two consecutive patients47 jointswith Crowe type â?¢ or â?£ were performed THA. Nine joints were Crowe type â?¢ and thirty eight joints were type â?£ in this study. All patients were female. The average age at the time of surgery was 63.3 years (range, 40–77 years). The average patient body weight was 50.5 kg and the average height was 150 cm (body mass index: 22.3 kg/m. 2). The average follow-up was 9.9 years (range, 1–21). See Table1 The socket was placed at the level of the original acetabulum, and femoral shortening osteotomy was performed in 27 joints. Leg lengthening (LL) was defined the vertical distance from the tip of greater trochanter to the tear drop line. The mean LL was 3.1 cm (range, 1.5–6.7 cm). Clinical Harris hip score and sciatic nerve palsy was retrospectively assessed from the patients records. The correlation between LL, the percent LL (cm) divided by body height (cm) (%LL/BH) and the incidence of sciatic nerve palsy was investigated. Results. There were two sciatic nerve palsy (4.3%). These leg lengthening were 5.2 cm and 6.7 cm, respectively. These patients had complete recovery of neurological function. A significant correlation between the amount of LL and the incidence of sciatic nerve palsy was identified. LL more than 5 cm and %LL/BH â?§ 3.6 were significant risk factors of sciatic nerve palsy (p=0.0194, p=0.0028). See table2.3 Body mass index, preoperative hip flexion, previous operations were not associated with sciatic nerve palsy. Harris hip scores improved from a mean of 57.6 points prior to surgery to an average of 83.1 points at the latest follow-up. Kaplan- Meier survivorship analysis at 5 and 10 years revealed a cumulative survival rate of 94.1% and 64.2% respectively, with any implant revision as the end point. There were six cases of dislocations, which were successfully treated conservatively. Infection was not observed. Conclusions. Leg lengthening more than 5 cm and %LL/BH â?§ 3.6 were significant risk factors of sciatic nerve palsy

(Case) 79-year-old woman. Past history, in 1989, right femur valgus osteotomy. in 1991, THA at left side. Follow-up thereafter. Hyaluronic acid injection for both knee osteoarthritis. (Clinical course)Her right hip pain getting worse and crawling indoors from the beginning of July 2013. We did right hybrid THA at August 2013(posterior approach, TridentHA cup, Exeter stem, Biolox Forte femoral head 28mm). But immediately, she dislocated twice than the third day after surgery because she became a delirium. It has been left by nurse for about 6 hours because of the midnight after the second dislocation. Next morning, check the dislocation limb position, closed reduction wasdone under intravenous anesthesia. As a result of waking up from the anesthesia, and complained of paralysis and violent pain in the right leg backward. A right lower extremity nerve findings, there is pain in the lower leg after surface about the calf, there was no apparent perception analgesia. Toe movement is weak, but the G-toe planter anddorsiflexion possible about M2, and neurological symptoms to relieved by flexion(above 70 degrees) of the right hip joint. Therefore, we thought that she suffered anterior dislocation of the sciatic nerve by the stem neck (retraction), judged to closed reduction was impossible, open reduction surgery was performed after waitingat hip flex position. But paralysis is gradually worsened during waiting surgery, toes movement had become impossible to operating room admission. Sciatic nerve is caught in front of the stem neck as expected, operative findings were able to finally reduction after removing the femoral head after dislocation. Anteversion of the cup was changed to 25 degrees from 15 degrees, and changed to 32mm diameter metal head and polyethylene liner. And we needed Intensive Care Unit(ICU) management after surgery for prevent recurrence of dislocation. Fitted with a hip brace for her, has not been re-dislocation. The sciatic nerve palsy improved in three months after the operation, the patient became able to walk without a cane. (Summary) We experienced a rare case suffered anterior dislocation of the sciatic nerve by the stem neck, and she had a good result after open reduction surgery

Hip resurfacing arthroplasty (HRA) is a bone conserving alternative to total hip arthroplasty. We present the early 1 and 2-year clinical and radiographical follow-up of a novel ceramic-on-ceramic (CoC) HRA in a multi-centric Australian cohort. Patient undergoing HRA between September 2018 and April 2021 were prospectively included. Patient-reported outcome measures (PROMS) in the form of the Forgotten Joint Score (FJS), HOOS Jr, WOMAC, Oxford Hip Score (OHS) and UCLA Activity Score were collected preoperatively and at 1- and 2-years post-operation. Serial radiographs were assessed for migration, component alignment, evidence of osteolysis/loosening and heterotopic ossification formation. 209 patients were identified of which 106 reached 2-year follow-up. Of these, 187 completed PROMS at 1 year and 90 at 2 years. There was significant improvement in HOOS (p< 0.001) and OHS (p< 0.001) between the pre-operative, 1-year and 2-years outcomes. Patients also reported improved pain (p<0.001), function (p<0.001) and reduced stiffness (p<0.001) as measured by the WOMAC score. Patients had improved activity scores on the UCLA Active Score (p<0.001) with 53% reporting return to impact activity at 2 years. FJS at 1 and 2-years were not significantly different (p=0.38). There was no migration, osteolysis or loosening of any of the implants. The mean acetabular cup inclination angle was 41.3° and the femoral component shaft angle was 137°. No fractures were reported over the 2-year follow-up with only 1 patient reporting a sciatic nerve palsy. There was early return to impact activities in more than half our patients at 2 years with no early clinical or radiological complications related to the implant. Longer term follow-up with increased patient numbers are required to restore surgeon confidence in HRA and expand the use of this novel product. In conclusion, CoC resurfacing at 2-years post-operation demonstrate promising results with satisfactory outcomes in all recorded PROMS

Nerve palsy occurring after elective primary total hip arthroplasty (THA) is a devastating complication because of its effect on motor strength, walking ability, potential for pain, and unexpected nature. In general, the nerve distribution involved is the peroneal branch of the sciatic nerve, and the level of involvement is usually mixed motor and sensory. Prior publications have associated limb lengthening, dysplasia and use of the posterior approach to be associated with a higher incidence of nerve palsy. In the literature, the incidence of sciatic nerve palsy is estimated to be 0.2 to 1.9%. We examined the rate of sciatic nerve palsy after THA performed by the joint replacement service at Hospital for Special Surgery between the years 1998–2013. Each case was matched with 2 controls that underwent THA and did not develop postoperative neuropathy. Controls were matched by surgical date having been within 7 days of their matched case's surgery date. Patient and surgical variables were reviewed using data from patient charts and the institution's total joint replacement registry. A multivariable logistic regression model was created to identify potential risk factors for neuropathy following THA while adjusting for potential confounders. We found that, of 39,056 primary THA cases, there were 81 cases of sciatic nerve palsy, giving an incidence of 0.21%. The factors with the greatest odds ratios for nerve palsy were: history of smoking (OR=3.45); history of spinal stenosis (OR=4.45), and time of day of 1PM or later (OR=3.98). We did not find limb lengthening, dysplasia, or type of fixation to be associated with nerve palsy. In conclusion, post-surgical neuropathy has a low incidence after primary THA, but at our institution, was associated with several factors. Spine-related comorbidities, such as spinal stenosis and lumbar spine disease, and smoking history should be closely monitored to inform the patient and surgeon for the potential increased risk of postoperative neuropathy following THA

Sciatic nerve injury remains a significant and devastating complication of total hip arthroplasty. Incidence as quoted in the literature ranges from 0.08% in primary joint replacement to 7.5% in revision arthroplasty. While as urgent exploration is recommended for nerve palsies associated with pain, management of sciatic nerve palsy with little or no pain is still controversial. In light of this, many patients with persistent palsies are often not referred to our specialist centre until after 6 months post-injury. The aim of this study was to review the outcomes of surgical intervention in patients presenting with sciatic nerve palsy more than 6 months after total hip arthroplasty. This retrospective cohort study identified 35 patients who underwent exploration and neurolysis of the affected sciatic nerve. All patients had documented follow-up at 1, 3, 6, 12 and 18 months to assess sensory and motor recovery. Patients were scored for sensory and motor function in the tibia and common personal nerve divisions, pre and post-operatively. The scoring system devised by Kline et al (1995) was used. Pre-operative electrophysiology was also reviewed. We found a statistically significant functional recovery following neurolysis of the sciatic nerve (p<0.01). A statistically significant relationship was also found between time to neurolysis and recovery of tibial nerve function (p = 0.02), such that greater delay to neurolysis was associated with poorer recovery. There was no significant relationship between time to neurolysis and recovery of common peroneal nerve function (p = 0.28). Our results indicate that the neurolysis of the sciatic nerve, six months or more post injury is associated with functional recovery. We feel that without surgical exploration this clinical improvement would not have occurred. Therefore, we believe that neurolysis plays a vital role at any stage of sciatic nerve injury. However, early presentation to a specialist unit is associated with better outcomes

Total hip arthroplasty (THA) in teenagers is uncommon and previously associated with poor survival rates. However it is sometimes the only option remaining to relieve pain and improve function in patients with advanced hip disease. We report on the clinical and radiological outcomes of THA in teenage patients. Medical records and radiographs of all consecutive teenage patients undergoing THA at a tertiary referral centre between 2006–2011 were reviewed. Mean follow-up was 3.4 years (range 0.6–6.8) with 9 patients having at least 5 years follow-up. Post-operative Harris hip, Oxford hip (OHS) and University of California Los Angeles (UCLA) activity scores were recorded. 51 THAs were performed in 43 patients (21 male, 22 female) with a mean age of 17 years (range 12–19). The 5 most common indications were slipped upper femoral epiphysis osteonecrosis 15 (29.4%), developmental hip dysplasia osteonecrosis 5 (9.8%), multiple/spondylo-epiphyseal dysplasia 5 (9.8%), chemotherapy-induced osteonecrosis 4 (7.8%) and idiopathic osteonecrosis 4 (8.2%). 46 (90%) were uncemented THAs and 5 (10%) were reverse hybrid THAs with 7 computer assisted design/manufacture (CADCAM) femoral implants. Articular bearings were ceramic/ceramic in 40 (78.4%), metal/metal 6 (11.8%), metal/polyethylene 3 (5.9%) and ceramic/polyethylene 2 (3.9%). The survival rate was 96% with 2 acute head revisions for 1 sciatic nerve palsy and 1 instability. Other complications (8.2%) included 1 dislocation, 1 sciatic nerve palsy that resolved, 1 surgical site infection and 1 unexplained pain. At latest follow-up, the mean Harris hip score was 90 (68–99), OHS was 42 (32–48) and UCLA activity score was 6 (4–9). Radiological analysis showed 2 patients with lucent lines around the acetabular component, but no signs of osteolysis or wear. As one of the largest studies on teenagers undergoing THA, we report good clinical and radiological outcomes at short to intermediate term follow-up

Uncontained acetabular defects with loss of superior iliac and posterior column support (Paprosky 3) represent a reconstructive challenge as the deficient bone will preclude the use of a conventional hemispherical cup. Such defects can be addressed with large metallic constructs like cages with and without allograft, custom tri-flange cups, and more recently with trabecular metal augments. An underutilised alternative is impaction bone grafting, after creating a contained cavitary defect with a reinforcement mesh. This reconstructive option delivers a large volume of bone while using a small-size socket fixed with acrylic cement. Between 2005 and 2014, 21 patients with a Paprosky 3B acetabular defect were treated with cancellous, fresh frozen impaction grafting supported by a peripheral reinforcement mesh secured to the pelvis with screws. A cemented all-polyethylene cup was used. Pre-operative diagnosis was aseptic loosening (15 cemented and 6 uncemented). The femoral component was revised in 10 patients. Post-operative course consisted of 3 months of protected weight bearing. Patients were followed clinically and radiographically. One patient had an incomplete post-operative sciatic palsy. After a mean follow up of 47 months (13 to 128) none of the patients required re-revision of the acetabular component. One asymptomatic patient presented with aseptic loosening 9 years post-operatively. Hardware failure was not observed. All patients had radiographic signs of graft incorporation and bone remodeling. There were no dislocations. The early and mid-term results of revisions of large acetabular defects with this technique are encouraging. Reconstitution of hip center of rotation and bone stock with the use of a small-size implant makes this technique an attractive option for large defects. Longer follow-up is needed to assess survivability

Introduction. Acetabular fractures management is controversial since, despite a good anatomical reduction, clinical outcome is not satisfactory very often and the probability of a total hip arthroplasty (THA) is high. Surgical treatment include long operating times, large approach, blood loss, neural and muscle damage, and a high risk of failure and secondary osteoarthritis related to bone necrosis, cartilage damage, and bone loss. We hypothesized that the acetabular fracture management affected the clinical and radiological outcome of THA after posttraumatic arthritis. Materials and Methods. We compared 49 patients (49 hips) initially treated conservatively followed some months later by THA in conjunction with acetabular reconstruction (group 1); and 29 patients (29 hips) who had undergone THA after a failed osteosynthesis (group 2). There were more associated fractures according to Letournel in group 2. The mean age was 59.3±15.8 years for group 1 and 52.9±15.2 years for group 2. The mean delay between fracture and THA was 75.4±5 months for group 1 and 59.4±5 for group 2. The mean follow-up was 11.7 in group 1 and 10.2 in group 2. Preoperative bone defect was similar. We used bone autograft in 13 hips (26.5%) in group 1 and four (13.6%) in group 2. We used acetabular reconstruction plates in 2 hips with a pelvic discontinuity in group 1. Complications, clinical outcome according to Harris Hip Score, and radiological reconstruction were compared. Two-way ANOVA with repeated measures were used for comparison. Results. There were 5 cups revised for aseptic loosening in group 1 and 2 in group 2. The cumulative probability of not having a cup revision at 16 years was 90.6% (95% confidence interval (CI) 78,1 to 100) for group 1 and 94.1% (95% CI 86.5 to 100) for group 2 (p=0.76). There were 4 sciatic palsies in group 2, 4 of which developed after trauma and 2 after osteosynthesis. There were no infections. Although pre-operative clinical score was better in group 1, post-operatively at latest follow-up there were no differences. The radiological reconstruction was better in group 1 for version angle (p=0.03) and abductor lever arm (p=0.02). The change from the pre- to post-operative situation was greater in group 2 for the latter (Delta value, p=0.002). The rate of post-operative heterotopic ossifications was greater in group 2 (p=0.04). Conclusions. Long-term clinical and radiograph results are good in patients who underwent THA after a complex acetabular fracture, however, primary THA in conjunction with acetabular reconstruction had a lower number of complications and a better radiographic acetabular reconstruction than a THA after failed osteosynthesis

The custom triflange acetabular component has been advocated for severe acetabular defects and pelvic discontinuity, cases in which a porous-coated hemisphere will not work. These are AAOS type III or IV defects, or alternatively classified as Paprosky 3B. Many have a pelvic discontinuity. A preoperative CT of the pelvis is sent to the manufacturer who generates a one to one scale 3D model of the hemipelvis. If the visualised defect cannot be treated with traditional methods then a triflanged component is created. Initial rigid fixation is obtained with screw fixation to the ilium and ischium. Subsequent bone ingrowth can provide long term fixation. The goal is to span the acetabular defect and obtain fixation to ilium and ischium with a third flange which rests on the pubis. Christie first reported on 67 hips (half with a discontinuity) with a mean follow-up of 53 months. No components were removed. There was an 8% reoperation for dislocation, 6% partial sciatic nerve palsy. Dennis reported 26 hips with a mean 54 month follow-up. Eighty-eight percent were considered successful. Taunton reported 57 cases with a pelvic discontinuity treated with a triflange at mean follow-up of 65 months. Eighty-one percent had a stable component and a healed pelvic discontinuity. The primary disadvantage of the technique is the preoperative time required to manufacture the device – typically 4–8 weeks

Introduction. The mid- or long-term results of acetabular revision total hip arthroplasty (THA) in Korea are rare. The purpose of this study is to report the mid-term radiographic results (> 5 years) of acetabular revision THA with porous-coated cementless Trilogy. ®. cup (Zimmer, Warsaw, IN, USA). Materials and Methods. Between 1999 and 2010, 77 patients (79 hips) had underwent acetabular revision THA with Trilogy. ®. cup. Eight patients (8 hips) were excluded due to death before 5-year follow-up, and 22 patients (23 hips) were excluded due to less than 5-year follow-up or follow-up loss. Forty-seven patients (48 hips) were included in our study. The mean age was 57.9 years (range, 36 to 76 years) and the mean follow-up was 9.8 years (range 5.0 to 16.2 years). The causes of revision were aseptic loosening in 40 hips, and septic loosening in 8 hips, respectively. Both acetabular and femoral revisions were performed in 14 hips and isolated acetabular revision was done in 34 hips. Preoperetive acetabular bone defect according to Paprosky classification was; 1 in type I, 6 in IIA, 11 in IIB, 9 in IIC, 15 in IIIA, and 6 in IIIB. Results. Radiolucent lines less than 2mm were found in 2 hips; one in zone I, another in zone I, II, III. Four hips (1 in type IIC, 1 in IIIA and 2 in IIIB) showed cup migration greater than 5 mm accompanying change of position greater than 5 degrees. However, these patients did not complain pain and showed fixation by secondary stabilization. The Kaplan-Meier survivorship with aseptic loosening as the end point at 10 years was 92.6% (95% confidence interval [CI], 82.6 – 100) and at 15 years was 83.8 % (95% CI, 69.1 – 98.6), respectively. Non-recurrent dislocations occurred in 4 hips. There were no other complication such as sciatic nerve palsy, infection and deep vein thrombosis. Conclusion. Mid-term radiographic results (>5 years) of acetabular revision THA with porous-coated cementless Trilogy. ®. cup showed durable longevity. However, other options such as anti-protrusio cage or cup-cage construct should be considered in severe acetabular bone defect

Introduction. Pelvic osteotomy such as Chiari osteotomy and rotational acetabular osteotomy (RAO) have been used successfully in patients with developmental dysplasia of the hip (DDH). However, some patients are forced to undergo total hip arthroplasty (THA) because of the progression of osteoarthritis. THA after pelvic osteotomy is thought to be more difficult because of altered anatomy of the pelvis. We compared six THAs done in dysplastic hips after previous pelvic osteotomy between 2008 and 2015 with a well-matched control group of 20 primary procedures done during the same period. Materials and methods. Six THAs for DDH after previous Pelvic osteotomy (three Chiari osteotomies and three RAOs) were compared with 20 THAs for DDH without previous surgery. The patients were matched for age, sex, and BMI. Minimum follow-up for both groups of patients was one year (range, 12–79 months and 12–77 months, respectively). The average interval from pelvic osteotomy to total hip arthroplasty was 19.8 years (range 12–26 years). Clinical and Radiological evaluations were performed. Results. Both groups had similar short-term results except clinical score. There were no signiï¬ï¿½cant differences in range of motion, intraoperative blood loss and operative time between the two groups. There were no infections, dislocations, intraoperative fractures, damaged nerves, or deep vein thromboses in either group. Clinical score according to Japanese Orthopaedic Association rating system at the most recent follow-up were significantly lower in previous pelvic osteotomy group (P=0.003). Lower clinical score was assumed to be caused by one patient with previous Chiari osteotomy who had contralateral sciatic nerve palsy which was unrelated to the surgery. Although we were concerned about the failures on the acetabular side in patients with THA after pelvic osteotomy because of osteosclerotic acetabular bone, no acetabular component exhibited loosening or revision. This result supports the thought that the pelvic osteotomy fragment maintains its blood supply and allows porous acetabular components to ingrow. Conclusion. Pelvic osteotomies do not seem to compromise the short-term clinical or radiographic outcome

The custom triflange acetabular component has been advocated for severe acetabular defects and pelvic discontinuity, cases in which a porous-coated hemisphere will not work. These are AAOS type III or IV defects, or alternatively classified as Paprosky 3B. Many have a pelvic discontinuity. A pre-operative CT of the pelvis is sent to the manufacturer who generates a one-to-one scale 3D model of the hemipelvis. The surgeon can review either a pdf file or an actual model. If the visualised defect cannot be treated with traditional methods then a triflanged component is created. The components have backside porous and hydroxyapatite coating. Initial rigid fixation is obtained with screw fixation to the ilium and ischium. Subsequent bone ingrowth can provide long term fixation. The goal is to span the acetabular defect and obtain fixation to the ilium and ischium with a third arm which rests on the pubis. Christie first reported on 67 hips (half with a discontinuity) with a mean follow-up of 53 months. No components were removed. There was an 8% reoperation for dislocation, 6% partial sciatic nerve palsy. 46% walked without support. Dennis reported 26 hips with a mean 54 month follow-up. Eighty-eight percent were considered successful. One implant was removed and left with a resection arthroplasty and 2 others had loose components but refused reoperation. Loosening of the ischial screws was a sign of failure in the three cases. Taunton reported 57 cases with a pelvic discontinuity treated with a triflange at mean follow-up of 65 months. Eighty-one percent had a stable component and a healed pelvic discontinuity. These authors also compared a custom triflange to a trabecular metal cup-cage construct finding similar implant costs of $12,500 and $11,250, respectively. All advocates of custom triflange acetabular components believe the results are similar or superior to other options in these very challenging cases at early follow-up. The primary disadvantage of the technique is the pre-operative time required to manufacture the device – typically 4–8 weeks

Total hip arthroplasty (THA) is frequently performed as a salvage procedure for the acetabular fracture when posttraumatic osteoarthritis, posttraumatic avascular necrosis, or fixation failure with subluxation develop. Special considerations for this situation include previous surgical exposure with dense scar tissue, the type and location of implants, the location and amount of heterotopic ossification, indolent infection, previous sciatic nerve palsy, and the pathoanatomy of existing acetabular defect. These factors can influence the choice of surgical exposure and the reconstructive method. The outcomes of THA after acetabular fracture are generally less favorable than those of the nontraumatic degenerative arthritis. Reason for this high failure is the low mean age and the high activity level of the patient. Other important reasons for failure include the problem of acetabular bone deficiency and compromised bone quality. We evaluated the results of cementless THA in patient who had previous acetabular fracture. We also compared this result with those of patients with posttraumatic avascular necrosis of the femoral head. Forty-five consecutive cementless THAs were performed for the treatment of post-traumatic osteoarthritis after acetabular fracture between December 1993 and December 2008. Of these patients, 15 patients were died or lost to follow-up monitoring before the end of the minimum one year follow-up period. This left 30 patients (30 hips) as the subjects of our retrospective review. We evaluated the clinical and radiographic results of these patients and compared with the results of THA in patients with post-traumatic AVN of the femoral head which had without acetabular damage. Two hips required revision of the cup secondary to early migration of the acetabular cup (1 hip) and postoperative deep infection (1 hip). There was no significant difference in clinical and radiographic results between two groups except implanted acetabular component size and required bone graft (p<0.05). The Kaplan-Meier ten-year survival rate, with revision as the end-point, was 90% and 96.7% with loosening of acetabular component as the end-point. Our series suggested that compared with cemented components, uncemented sockets may improve the results of arthroplasty after previous acetabular fracture. In conclusion, cementless THA following acetabular fracture presents unique challenge to the surgeon, careful preoperative assessment and secure component fixation with proper bone grafting is essential to minimize problems

Background. Revision THA presents significant challenges for the surgeon when the proximal femur is deficient or mechanically unreliable. The aim of this study is to assess the clinical and functional results of the use of tumor enndoprosthesis to reconstruct the proximal femur when there is massive bone loss. Patients and Methods. A prospective study was conducted involving 10 cases. The follow up of the cases ranged from 12 months to 30 months with a mean period of an average of 23months. The indications for revision surgery were aseptic loosening in 9 cases and septic loosening in one case Harris hip score was used for pre and postoperative clinical evaluation of the patients. Results. At the latest follow up the Harris Hip scores improved from a preoperative average of 16 (range, 3-47), to a postoperative average of 75.6 (range, 66-94). The complications that we encountered in the study included one case of superficial wound infection, another case developed sciatic nerve palsy postoperatively. No other complications were reported. Conclusion. Revision hip replacement in proximally compromised femurs presents a significant surgical challenge. When there is massive proximal femoral bone loss proximal fitting revision stems do not achieve adequate fixation hence the use of tumor prosthesis is indicated

Failed internal fixation of hip fracture is a problem with varied aetiology. This becomes more complex when associated with infection. Total hip arthroplasty (THA) remains the only option to restore hip biomechanics when there is partial/complete head destruction associated with it. A retrospective review was performed for 22 consecutive patients of THA following failed infected internal fixation between Sept. 2001 and Nov. 2007. There were 11 dynamic hip screw failures for intertrochanteric fractures, six failed osteotomies following proximal femoral fractures and five failed screw fixations for transcervical fractures. The average age of the patients was 48.5 years and average follow up period was 3.5 years (16 months–7.5 years). All the patients have undergone two stage revision surgeries. The average Harris Hip Score improved from 35.5 to 82.8 at the latest follow up. None of the patients had recurrence of infection. One patient developed sciatic nerve palsy, recovered partially at one year following surgery. The results were comparable to primary arthroplasty in femoral neck fractures. THA is a useful salvage procedure for failed infected internal fixation of hip fractures. Extreme care must be taken to avoid fracture and penetration of femoral shaft in such cases. Auto graft, allograft and special components like multihole cup, narrow stem should be available for reconstruction in difficult cases

We retrospectively reviewed 54 patients (92 hips) who underwent cementless total hip arthroplasty for bony ankylosis in ankylosing spondylitis between September 1988 and 2002. Clinical assessment was done at follow-up, which envisages assessment of the pain, function, deformities and range of motion using the Harris Hip Score. Radiographic analysis was done. Kaplan-Meier survivorship analysis was done at 5 and 8.5 years using the revision for the removal of femoral component, acetabular component or both due to any cause as the end point. The mean age of the patients was 25.5 years. The mean duration of follow up was 8.5 years. The average preoperative Harris Hip Score of 49.5 improved to 82.6 post operatively. Post operatively 10 hips had mild to moderate pain. Anterior dislocation occurred in four hips (4.3 %) and sciatic nerve palsy in one hip. Heterotopic ossification was seen in 12 patients, reankylosis rate was 0%. Thirteen arthroplasties were revised due to aseptic loosening. Kaplan-Meier survivorship analysis with revision as end point revealed 98.8% survival at 5 years and 85.8% survival at 8.5 years 11 follow up. Cementless THA in osseous ankylosis in ankylosing spondylitis is a worthwhile surgical intervention in bony ankylosis. Newfound mobility, manoeuvrability and improved ability to sit comfortably were the outcomes, which alleviated the patients’ daunted morale. However, the technically demanding nature of the procedure should not be underestimated

Restoration of an anatomical hip centre frequently requires limb lengthening, which increases the risk of nerve injury in the treatment of Crowe 4 DDH. The objective was to perform a prospective evaluation of SDTSO with Cemented Exeter Femoral Component. 15 female patients (18 hips – 3 bilateral) with a mean age at time of operation of 51 years were followed-up for a mean of 77 months (11 to 133). 16 cemented and 2 uncemented acetabular components were implanted. Exeter cemented DDH stems were used in all cases. No patient was lost to follow-up. Charnley-d'Aubigné-Postel scores for pain, function and range of movement were improved from a mean of 2, 2, 3 to 5, 4, 5 respectively. One osteotomy failed to unite at 14 months and was revised successfully. Clinical healing was achieved at a mean of 6 months and radiological at a mean of 9 months. The mean length of the excised segment was 3cm and the mean true limb lengthening was 2cm. A 3.5mm DCP plate with unicortical screws was used to reduce the osteotomy, and intramedullary autografting was performed in all cases. Mean subsidence was 1mm and no stem was found loose at the latest follow-up. No sciatic nerve palsy was observed and no dislocation. Cemented Exeter femoral components perform well in the treatment of Crowe IV DDH with SDTSO. Transverse osteotomy is necessary to achieve derotation and reduction can be maintained with a DCP plate. Intramedullary autografting prevents cement interposition at the osteotomy site
