Removal of bullets retained within joints is indicated to prevent mechanical blockade, 3rd body wear and resultant arthritis, plus lead arthropathy and systemic lead poisoning. The literature is sparse on this subject, with mostly sporadic case reports utilizing hip arthroscopy. We report on the largest series of removal of bullets from the hip joints using open surgical. We reviewed prospectively collected data of patients who presented to a single institution with civilian gunshot injuries that breached the hip joint between 01 January 2009 and 31 December 2022. We included all cases where the bullet was retained within the hip joint area. Exclusion criteria: cases where the hip joint was not breached, bullets were not retained around the hip area or cases with isolated acetabulum involvement. One hundred and eighteen (118) patients were identified. One patient was excluded as the bullet embedded in the femur neck was sustained 10 years earlier. Of the remaining 117 patients, 70 had retained bullets around the hip joint. Aims. Accurate positioning of the acetabular component is essential for achieving the best outcome in total hip arthroplasty (THA). However, the acetabular shape and anatomy in severe hip dysplasia (Crowe type IV hips) is different from that of arthritic hips. Positioning the acetabular component in the acetabulum of Crowe IV hips may be surgically challenging, and the usual surgical landmarks may be absent or difficult to identify. We analyzed the acetabular morphology of Crowe type IV hips using CT data to identify a landmark for the ideal placement of the centre of the acetabular component as assessed by morphometric geometrical analysis and its reliability. Patients and Methods. A total of 52 Crowe IV and 50 normal hips undergoing total hip arthroplasty were retrospectively identified. In this CT-based simulation study, the acetabular component was positioned at the true acetabulum with a radiographic inclination of 40° and anteversion of 20° (Figure 1). Acetabular shape and the position of the centre of the acetabular component were analyzed by morphometric geometrical analysis using the generalized Procrustes analysis (Figure 2). To describe major trends in shape variations within the sample, we performed a principal component analysis of partial warp variables (Figure 3). Results. The plot of the landmarks showed that the centre of the acetabular component of normal hips was positioned around the centre of the acetabulum and superior and slightly posterior on the acetabular fossa (Figure 3).

Custom 3D printed implants can be anatomically designed to assist in complex surgery of the bony pelvis in both orthopaedic oncology and orthopaedic reconstruction surgery. This series includes patients who had major pelvic bone loss after initially presenting with tumours, fractures or infection after previous total hip arthroplasty. The extent of the bone loss in the pelvis was severe and therefore impossible to be reconstructed by conventional ‘off –the-shelve’ implants. The implant was designed considering the remaining bony structures of the contra-lateral hemi- pelvis, to provide an anatomical, secured support for the reconstructed hip joint. The latter was realised by strategically orientated screws and by porous structures (an integral part of the implant), which stimulates osseointegration. A custom pelvic implant was designed, manufactured and 3D printed. Reconstruction of the pelvis was performed together with a cemented (bipolar bearing) acetabular cup. In some cases, a proximal femoral replacement was also necessary to compensate for bony defects. All patients had sufficient range of motion (ROM) at the hip with post-operative stability. This retrospective study analyzes all acetabular fractures in patients over the age of 60, managed by ORIF at a tertiary trauma care centre between 2007 and 2018 with a minimum follow up of one year. Of the 117 patients reviewed, 85 patients undergoing ORIF for treatment of their acetabular fracture were included in the analysis. The remainder were excluded based management option including acute ORIF with THA (n=10), two-stage ORIF (n=2), external fixator only (n=1), acute THA (n=1), and conservative management (n=1). Introduction. The incidence of acetabular fractures in the elderly population is increasing. Treatment with staged or acute total hip arthroplasty (THA) is occasionally required. The role of acute THA however, remains controversial. The purpose of our study was to assess the outcomes of a subgroup of elderly patients who underwent early simultaneous open reduction and internal fixation (ORIF) and primary THA for displaced acetabular fractures. Materials and Methods. 86 patients underwent ORIF for displaced acetabular fractures at The Alfred Hospital, Melbourne between August 2007 and August 2009. Eight of these patients underwent early simultaneous ORIF and primary THA. Mean age was 79 years. Mean time between injury and surgery was 4 days. Mean time of follow-up was 19 months. Introduction. Acetabular fractures are a challenging problem. It has been published that outcome is dependent upon the type of fracture, the reduction of the fracture and concomitant injuries. The end-points of poor outcome include avascular necrosis of the femoral head, osteoarthritis. However, we lack definitive statistics and so counselling patients on prognosis could be improved. In order to achieve this, more outcome studies from tertiary referral centres are required. We present the first long term follow up from a large tertiary referral Centre in Ireland. Methods. We identified all patients who were ten years following open reduction and internal fixation of an acetbular fracture in our centre. We invited all of these patients to attend the hospital for clinical and radiographic follow-up. As part of this, three scoring systems were completed for each patient; the Short-form 36 health survey (SF36), the Merle d'Aubigné score and the Short Musculoskeletal Functional Assessment (SMFA). Results. The data represents one years activity at a new tertiary referral unit. We idenfied a total of 44 patients who were ten years following ORIF of acetabular fractures in our unit. 21 patients (48%) replied to written invitation and attended the hospital for clinical and radiographic follow-up. A further 7 patients were contacted by telephone and interviewed to guage their rehabilitation. 3 patients had passed away. The remaining 13 patients were not contactable. Of those who attended in person for follow-up; 18 were male and 3 were female. The mean age at follow-up was 40.5 years (Range 27-60). To define the common pathology of the hips with irreducible posterior dislocation combined with femoral head fracture and the outcome of surgical treatment using surgical hip dislocation technique. Introduction. The ESP prosthesis is a one-piece deformable but cohesive interbody spacer. it provides 6 full degrees of freedom about the 3 axes including shock absorption (fig1). The prosthesis geometry allows limited rotation and translation with resistance to motion (elastic return property) aimed at avoiding overload of the posterior facets. The rotation center can vary freely during motion in this “silentblock” implant. It thus differs substantially from current prostheses. Material and methods. Surgeries were performed by 2 senior surgeons in 54 women and 34 men (1level in 72 cases, 2 levels in 3 cases, hybrid construct in 13 cases). Average age was 42 (SD: 7). Average BMI was 24.2kg/m2 (SD: 3,4). Clinical data and X-rays were collected at the preoperative time and at 3, 6, 12, 24, and 60 months post-op. The analysis was performed by a single observer independent from the selection of patients and from the surgical procedure. The radiological analysis at 60 months follow-up could be realized in only 76 cases because the quality of the dynamic Xrays was not sufficient in 12 patients.
