The purpose of this study was to assess the clinical and radiological outcome following spring plate fixation of posterior wall fractures. Spring plates are fashioned from a one third tubular plate cutting through one of the holes in the plate and bending the sharp edges through 90 degrees allowing screw fixation of small acetabular margin fragments well away from the joint reducing the risk of joint penetration. From July 1993 to August 2004, 89 patients with displaced posterior wall fractures underwent posterior wall fixation with one or more spring plates. Patients were assessed post-operatively with a CT scan and annually for up to 5 years for a clinical and radiological assessment. Clinically patients were graded according to the Epstein modification of Merle d'Aubigné/Postel Hip Score. The radiographs were graded using the Roentographic Grade criteria used by Matta. Patients were reviewed at a mean 55 month follow-up. There were 12 post-operative complications. Clinically excellent or good results were seen in 70% and radiologically in 70%. There were 15 revisions for osteonecrosis, infection and osteoarthritis. 91% (20/22) of fractures had excellent/good clinical results if reduced anatomically compared with 66% (24/36) of those reduced to within 2mm as assessed by the post-operative CT scan. Only 44% (8/18) of those reduced leaving a gap of greater than 2mm had an excellent/good clinical result. Posterior wall fractures can be treated successfully by the use of spring plates. Clinical results correspond closely with radiological appearance. The accuracy of reduction correlates highly with the subsequent prognosis and we recommend routine post-operative CT scanning to identify misplaced metalwork and the accuracy of reduction to help predict prognosis

Stable, anatomical fixation of acetabular fractures gives the best chance of a good outcome. We performed a biomechanical study to compare fracture stability and construct stiffness of three methods of fixation of posterior wall acetabular fractures. Two-dimensional motion analysis was used to measure fracture fragment displacement and the construct stiffness for each fixation method was calculated from the force / displacement data. Following 2 cyclic loading protocols of 6000 cycles, to a maximum 1.5kN, the mean fracture displacement was 0.154mm for the rim plate model, 0.326mm for the buttress plate and 0.254mm for the spring plate model. Mean maximum displacement was significantly less for the rim plate fixation than the buttress plate (p=0.015) and spring plate fixation (p=0.02). The rim plate was the stiffest construct 10962N/mm (SD 3351.8), followed by the spring plate model 5637N/mm (SD 832.6) and the buttress plate model 4882N/mm (SD 387.3). Where possible a rim plate with inter-fragmentary lag screws should be used for isolated posterior wall fracture fixation as this is the most stable and stiffest construct. However, when this method is not possible, spring plate fixation is a safe and superior alternative to a posterior buttress plate method

Removal of bullets retained within joints is indicated to prevent mechanical blockade, 3rd body wear and resultant arthritis, plus lead arthropathy and systemic lead poisoning. The literature is sparse on this subject, with mostly sporadic case reports utilizing hip arthroscopy. We report on the largest series of removal of bullets from the hip joints using open surgical. We reviewed prospectively collected data of patients who presented to a single institution with civilian gunshot injuries that breached the hip joint between 01 January 2009 and 31 December 2022. We included all cases where the bullet was retained within the hip joint area. Exclusion criteria: cases where the hip joint was not breached, bullets were not retained around the hip area or cases with isolated acetabulum involvement. One hundred and eighteen (118) patients were identified. One patient was excluded as the bullet embedded in the femur neck was sustained 10 years earlier. Of the remaining 117 patients, 70 had retained bullets around the hip joint. In 44 patients we undertook bullet removal using the followingsurgical hip dislocation (n = 18), hip arthrotomy (n = 18), removal at site of fracture fixation/replacement (n = 2), posterior wall osteotomy (n = 1), direct removal without capsulotomy (tractotomy) (n = 5). In 26 patients we did not remove bullets for the following reasons: final location was extra-capsular embedded in the soft tissues (n=17), clinical decision to not remove (n=4), patients’ clinical condition did not allow for further surgery (n= 4) and patient refusal (n=1). No patients underwent hip arthroscopy. With adequate pre-operative imaging and surgical planning, safe surgical removal of retained bullets in the hip joint can be achieved without the use of hip arthroscopy; using the traditional open surgical approaches of arthrotomy, tractotomy and surgical hip dislocation

Femoroacetabular impingement (FAI) – enlarged, aspherical femoral head deformity (cam-type) or retroversion/overcoverage of the acetabulum (pincer-type) – is a leading cause for early hip osteoarthritis. Although anteverting/reverse periacetabular osteotomy (PAO) to address FAI aims to preserve the native hip and restore joint function, it is still unclear how it affects joint mobility and stability. This in vitro cadaveric study examined the effects of surgical anteverting PAO on range of motion and capsular mechanics in hips with acetabular retroversion. Twelve cadaveric hips (n = 12, m:f = 9:3; age = 41 ± 9 years; BMI = 23 ± 4 kg/m2) were included in this study. Each hip was CT imaged and indicated acetabular retroversion (i.e., crossover sign, posterior wall sign, ischial wall sign, retroversion index > 20%, axial plane acetabular version < 15°); and showed no other abnormalities on CT data. Each hip was denuded to the bone-and-capsule and mounted onto a 6-DOF robot tester (TX90, Stäubli), equipped with a universal force-torque sensor (Omega85, ATI). The robot positioned each hip in five sagittal angles: Extension, Neutral 0°, Flexion 30°, Flexion 60°, Flexion 90°; and performed hip internal-external rotations and abduction-adduction motions to 5 Nm in each position. After the intact stage was tested, each hip underwent an anteverting PAO, anteverting the acetabulum and securing the fragment with long bone screws. The capsular ligaments were preserved during the surgery and each hip was retested postoperatively in the robot. Postoperative CT imaging confirmed that the acetabular fragment was properly positioned with adequate version and head coverage. Paired sample t-tests compared the differences in range of motion before and after PAO (CI = 95%; SPSS v.24, IBM). Preoperatively, the intact hips with acetabular retroversion demonstrated constrained internal-external rotations and abduction-adduction motions. The PAO reoriented the acetabular fragment and medialized the hip joint centre, which tightened the iliofemoral ligament and slackenend the pubofemoral ligament. Postoperatively, internal rotation increased in the deep hip flexion positions of Flexion 60° (∆IR = +7°, p = 0.001) and Flexion 90° (∆IR = +8°, p = 0.001); while also demonstrating marginal decreases in external rotation in all positions. In addition, adduction increased in the deep flexion positions of Flexion 60° (∆ADD = +11°, p = 0.002) and Flexion 90° (∆ADD = +12°, p = 0.001); but also showed marginal increases in abduction in all positions. The anteverting PAO restored anterosuperior acetabular clearance and increased internal rotation (28–33%) and adduction motions (29–31%) in deep hip flexion. Restricted movements and positive impingement tests typically experienced in these positions with acetabular retroversion are associated with clinical symptoms of FAI (i.e., FADIR). However, PAO altered capsular tensions by further tightening the anterolateral hip capsule which resulted in a limited external rotation and a stiffer and tighter hip. Capsular tightness may still be secondary to acetabular retroversion, thus capsular management may be warranted for larger corrections or rotational osteotomies. In efforts to optimize surgical management and clinical outcomes, anteverting PAO is a viable option to address FAI due to acetabular retroversion or overcoverage

Aims. Accurate positioning of the acetabular component is essential for achieving the best outcome in total hip arthroplasty (THA). However, the acetabular shape and anatomy in severe hip dysplasia (Crowe type IV hips) is different from that of arthritic hips. Positioning the acetabular component in the acetabulum of Crowe IV hips may be surgically challenging, and the usual surgical landmarks may be absent or difficult to identify. We analyzed the acetabular morphology of Crowe type IV hips using CT data to identify a landmark for the ideal placement of the centre of the acetabular component as assessed by morphometric geometrical analysis and its reliability. Patients and Methods. A total of 52 Crowe IV and 50 normal hips undergoing total hip arthroplasty were retrospectively identified. In this CT-based simulation study, the acetabular component was positioned at the true acetabulum with a radiographic inclination of 40° and anteversion of 20° (Figure 1). Acetabular shape and the position of the centre of the acetabular component were analyzed by morphometric geometrical analysis using the generalized Procrustes analysis (Figure 2). To describe major trends in shape variations within the sample, we performed a principal component analysis of partial warp variables (Figure 3). Results. The plot of the landmarks showed that the centre of the acetabular component of normal hips was positioned around the centre of the acetabulum and superior and slightly posterior on the acetabular fossa (Figure 3). The acetabular shapes of Crowe IV hips were distinctively triangular; the ideal position of the centre of the acetabular component was superior on the posterior bony wall (Figure 3). The first and second relative warps explained 34.2% and 18.4% of the variance, respectively, compared with that of 28.6% and 18.0% in normal hips. We defined the landmark as one-third the distance from top on the posterior bony wall in Crowe IV hips. The average distance from the centre of the acetabular component was 5.6 mm. There were 24 hips (50%) for which the distance from 1/3 pbw was within 5 mm, and 43 hips (89.6%) for which the distance was within 10 mm. Conclusions. Morphometric geometrical analysis showed that the acetabulum shape of Crowe type IV hips was distinctively triangular; the centre of the acetabular component was not positioned at the centre of the acetabulum, but rather superior on the posterior bony wall. The point one-third from the top on the posterior bony wall was a useful landmark for surgeons to set the acetabular component in the precise position in Crowe IV hips. Different factors associated with an adverse functional outcome of acetabular fractures involving the posterior wall have been well documented. Among these is marginal impaction. From 1998 until mid-2006, 105 cases were prospectively reviewed to assess the functional and radiological results of simple and complex acetabular fractures associated with a posterior wall component, with special reference to the marginal impaction. Associated posterior wall components associated with marginal impaction were compared to the pattern of fractures without marginal impaction. The exclusion criteria were non-anatomic reductions i.e. > 2 mm displacement, avascular necrosis, deep infection, heterotropic ossification grade III, IV, chondrolysis and nerve injuries. All marginal impaction fractures were identified on the pre-operative CT scan. They were openly reduced, elevated and autografted from the greater trochanter, followed by rigid internal fixation for early post-operative mobilisation i.e. CPM use. Results were assessed clinically by the modified Merle d'Aubigné and Postel score and radiologically by Kellgren and Laurence method of grading of osteoarthritis. 40 cases were excluded for the reasons above. This left 27 cases of marginal impaction and 38 cases of control. The average period of follow-up was 35.7 months. The functional results of the marginal impaction grafting group revealed 13 (48.1%) excellent, 7 (25.9%) very good, 6 (22.2%) good and one (3.8%) fair, while the control group showed 18 (47.4%) excellent, 10 (26.3) very good, 8 (21%) good, one (2.6%) fair and one (2.6%) poor result. The functional and radiological outcome of the posterior wall component fractures associated with marginal impaction showed very satisfactory results in comparison with a control group in the short and mid-term period. An integral part of this is careful recognition of this injury pattern and its management as part of the open reduction and fixation

Custom 3D printed implants can be anatomically designed to assist in complex surgery of the bony pelvis in both orthopaedic oncology and orthopaedic reconstruction surgery. This series includes patients who had major pelvic bone loss after initially presenting with tumours, fractures or infection after previous total hip arthroplasty. The extent of the bone loss in the pelvis was severe and therefore impossible to be reconstructed by conventional ‘off –the-shelve’ implants. The implant was designed considering the remaining bony structures of the contra-lateral hemi- pelvis, to provide an anatomical, secured support for the reconstructed hip joint. The latter was realised by strategically orientated screws and by porous structures (an integral part of the implant), which stimulates osseointegration. A custom pelvic implant was designed, manufactured and 3D printed. Reconstruction of the pelvis was performed together with a cemented (bipolar bearing) acetabular cup. In some cases, a proximal femoral replacement was also necessary to compensate for bony defects. All patients had sufficient range of motion (ROM) at the hip with post-operative stability. It has been verified, at six and twelve months postoperatively, that there is a strong hold of the implant due to osseointegration. Additionally, in patients whose posterior acetabular wall was missing, it was discovered that the implant assisted in bone formation and covered the entire posterior surface of the implant. All patients in this study managed with this novel treatment option, proved to have a stable pelvic reconstruction with restoration of leg lengths, improvement of strength and independent ambulation at short and medium term follow-up

Introduction. Acetabular retroversion is an accepted cause of Pincer-type femoroacetabular impingement. There is increasing evidence that acetabular retroversion is rather a rotational abnormality of the pelvis than an overgrowth of the acetabular wall or even a dysplasia of the posterior wall. Initially, patients with a retroverted acetabulum were treated with an open rim trimming through a surgical hip dislocation (SHD) based on the early understanding of the pathomorphology. Theoretically, the reduction of the anterior wall can decrease the already small joint contact area in retroverted hips to a critical size. Based on the most recent literature, anteverting periacetabular osteotomy (PAO) seems to be the more appropriate surgical treatment. With this technique, the anterior impingement conflict can be treated efficiently without compromising the joint contact area. However, it is unknown whether this theoretical advantage in turn results in better mid term results of treatment. Objectives. We asked if anteverting PAO results in better clinical and radiographical mid term results compared to rim trimming through a surgical hip dislocation. Methods. We performed a retrospective comparative study based on 257 hips (221 patients) with symptomatic femoroacetabular impingement due to acetabular retroversion. Acetabular retroversion was defined by a cross-over sign, a posterior wall sign, and a positive ischial spine sign. We then formed two matched groups consisting of 73 hips undergoing open acetabular rim trimming and 54 hips with an anteverting periacetabular osteotomy. Patients with incomplete radiographic documentation, previous surgery, and hip dysplasia (LCE < 20°) were excluded. Some patients were excluded due to a matching process (Fig. 1). Patients were generally followed clinically and radiographically after two, five and ten years. A Kaplan-Meier survivorship analysis was performed using the following endpoints: a low clinical score, radiographic progression of osteoarthritis, and/or the conversion to total hip arthroplasty. The Log Rank Score was used to compare the two survivorship curves. Results. Patients undergoing anteverting PAO for acetabular retroversion had a significantly increased survivorship (82%, 95% confidence interval, 72–91%) at seven years in comparison to open surgical rim trimming (63%, 95%CI, 49–76%, p<0.0001). The two survivorship curves are comparable for the first four years with a substantial drop for the rim trimming group after year five (Fig. 2). Conclusion. This study proofs for the first time that the theoretical advantages of anteverting periacetabular osteotomy in hips with symptomatic acetabular retroversion results in an increased survivorship at mid term follow-up in comparison to open rim trimming. Management of acetabular fractures in the elderly population remains somewhat controversial in regards to when to consider is open reduction internal fixation (ORIF) versus acute primary total hip. study aims to (1) describe outcome of this complex problem and investigate predictors of successful outcome. This retrospective study analyzes all acetabular fractures in patients over the age of 60, managed by ORIF at a tertiary trauma care centre between 2007 and 2018 with a minimum follow up of one year. Of the 117 patients reviewed, 85 patients undergoing ORIF for treatment of their acetabular fracture were included in the analysis. The remainder were excluded based management option including acute ORIF with THA (n=10), two-stage ORIF (n=2), external fixator only (n=1), acute THA (n=1), and conservative management (n=1). The remainder were excluded based on inaccessible medical records (n=6), mislabelled diagnosis (n=6), associated femoral injuries (n=4), acetabular fracture following hemiarthroplasty (n=1). The mean age of the cohort is 70±7 years old with 74% (n=62) of patients being male. Data collected included: demographics, mechanism of injury, Charlson Comorbidity Index (CCI), ASA Grade, smoking status and reoperations. Pre-Operative Radiographs were analyzed to determine the Judet and Letournel fracture pattern, presence of comminution and posterior wall marginal impaction. Postoperative radiographs were used to determine Matta Grade of Reduction. Outcome measures included morbidity-, mortality- rates, joint survival, radiographic evidence of osteoarthritis and patient reported outcome measures (PROMs) using the Oxford Hip Score (OHS) at follow-up. A poor outcome in ORIF was defined as one of the following: 1) conversion to THA or 2) the presence of radiographic OA, combined with an OHS less than 34 (findings consistent with a hip that would benefit from a hip replacement). The data was analyzed step-wise to create a regression model predictive of outcome following ORIF. Following ORIF, 31% (n=26) of the cohort had anatomic reduction, while 64% (n=54) had imperfect or poor reduction. 4 patients did not have adequate postoperative radiographs to assess the reduction. 31 of 84 patients undergoing ORIF had a complication of which 22.6% (n=19) required reoperation. The most common reason being conversion to THA (n=14), which occurred an average of 1.6±1.9 years post-ORIF. The remainder required reoperation for infection (n=5). Including those converted to THA, 43% (n=36) developed radiographic OA following acetabular fracture management. The mean OHS in patients undergoing ORIF was 36 ± 10; 13(16%) had an OHS less than 34. The results of the logistic regression demonstrate that Matta grade of reduction (p=0.017), to be predictive of a poor outcome in acetabular fracture management. With non-anatomic alignment following fixation, patients had a 3 times greater risk of a poor outcome. No other variables were found to be predictive of ORIF outcome. The ability to achieve anatomic reduction of fracture fragments as determined by the Matta grade, is predictive of the ability to retain the native hip with acceptable outcome following acetabular fracture in the elderly. Further research must be conducted to determine predictors of adequate reduction in order to identify candidates for ORIF

Fracture of the acetabulum can lead to degenerative arthritis of the hip, avascular necrosis of the femoral head, or both. Total hip arthroplasty is a common form of surgical treatment when significant joint changes and pain are present. Ten patients with fracture acetabulum were treated in this study using metal on metal total hip arthoplasty. The initial fracture was posterior wall fracture in one patient, posterior column fracture in one patient, transverse fracture in 2 patients, fracture dislocation in 3 patients and fracture posterior wall and column in 3 patients. The indications of arthroplasty were secondry osteoarthritis after internal fixation or after conservative management or collapse of the femoral head. Arthroplasty was done after an average period of 1.8 years (range from 1 to 4 years). After a follow up period ranged from 3 to 7 years with a mean of 4.6 years, the Harris hip score was improved from a mean of 51 (range 20 to 65) to a mean of 92.5 (range 90 to 95). Infection occurred in one case and two stages revision was done. Another case developed loosening of the acetabular component and was revised using cementless cup fixed with screws and bone graft. Metal on metal THR after acetabular fracture are relatively uncomplicated and lead to a good outcome despite the difficulties faced during the procedure

Few epidemiological studies from Asian countries have addressed this issue and reported that FAI is less prevalent in Asian population. The purpose of this study was to determine the prevalence of radiographic hip abnormalities associated with FAI in asymptomatic Korean volunteers. The authors hypothesized that the prevalence of FAI in Korean population would not be less than that in western population. Two hundred asymptomatic volunteers with no prior hip surgery or childhood hip problems underwent three-view plain radiography (pelvis anteroposterior (AP) view, Sugioka view, and 45° Dunn view) of both hips. Cam lesions were defined as the presence of the following signs on each views: pistol-grip deformity, osseous bump at the femoral head-neck junction, flattening of the femoral head-neck offset, or alpha angle >50°. Pincer lesions were determined by radiographic signs, including crossover sign, posterior wall deficient sign, or lateral center-edge (CE) angle >40°. Only positive cases agreed by both observers were defined as true FAI-related deformities. There were 146 male and 254 female hips, with a mean age of 34.7 years. On pelvis AP view, the prevalence of pistol grip deformity, bump, flattening, and alpha angle >50° was 1.3% (male 3.4%, female 0%), 0.8% (male 2.1%, female 0%), 0.8% (male 2.1%, female 0%), and 1.0% (male 2.7%, female 0%), respectively. On Sugioka view, the prevalence of bump, flattening, and alpha angle >50° was 9.8% (male 14.4%, female 7.1%), 13.5% (male 20.5%, female 9.4%), and 14.0% (male 26.7%, female 6.7%), respectively. On 45° Dunn view, the prevalence of bump, flattening, and alpha angle >50° was 8.0% (male 14.4%, female 4.3%), 17.5% (male 27.4%, female 11.8%), and 27.5% (male 44.5%, female 17.7%), respectively. The prevalence of cam lesion which was identified on at least one radiograph was 42.5% (male 62.3%, female 31.1%). The prevalence of cam lesion which was identified on ≥2 radiographs was 19.3% (male 30.8%, female 12.6%). The prevalence of cam type FAI (at least one cam lesion) was 2.0% (male 5.5%, female 0%) on pelvis AP view, 25.8% (male 37.0%, female 19.3%) on Sugioka view, and 35.8% (male 55.5%, female 24.4%) on 45° Dunn view. On pelvis AP view, the prevalence of crossover sign, posterior wall sign, and CE angle >40° was 20.0% (male 23.3%, female 18.1%), 20.8% (male 22.6%, female 19.7%), and 2.0% (male 2.7%, female 1.6%), respectively. The prevalence of pincer type of FAI (at least one pincer lesion) was 23.0% (male 27.4%, female 20.5%). In asymptomatic Korean volunteers, the prevalence of cam type FAI was low on AP pelvis radiographs, whereas the prevalence of cam type FAI on Sugioka and 45° Dunn view was found to be comparable to that previously reported in Western populations. The prevalence of pincer type FAI in asymptomatic Korean volunteers was also comparable that in Western populations and was similar in both gender. Considering the high prevalence of FAI morphologic features on plain radiographs in asymptomatic Korean populations, it is also important to determine whether FAI is a cause of hip pain when considering surgery in Asian patients

Introduction. The incidence of acetabular fractures in the elderly population is increasing. Treatment with staged or acute total hip arthroplasty (THA) is occasionally required. The role of acute THA however, remains controversial. The purpose of our study was to assess the outcomes of a subgroup of elderly patients who underwent early simultaneous open reduction and internal fixation (ORIF) and primary THA for displaced acetabular fractures. Materials and Methods. 86 patients underwent ORIF for displaced acetabular fractures at The Alfred Hospital, Melbourne between August 2007 and August 2009. Eight of these patients underwent early simultaneous ORIF and primary THA. Mean age was 79 years. Mean time between injury and surgery was 4 days. Mean time of follow-up was 19 months. There were 3 both-column fractures, 2 anterior column, 1 posterior wall, 1 transverse with posterior wall and 1 T-shaped. Two patients had an associated neck of femur fracture and two had an impaction fracture of the femoral head. The Harris and Oxford hip scores were used to assess clinical outcome. Radiographs were analysed for component loosening. Results. There was one unrelated post-operative death at 5 months. There was a high rate of post-operative complications. Four patients developed heterotopic ossification, 2 extensive. There was one superficial and one deep infection. One patient has a persistent post-operative foot drop. The Harris hip scores ranged from 45 to 86 with a mean of 68. The Oxford hip scores ranged from 24 to 37 with a mean of 32. There was no evidence of acetabular component loosening. Conclusion. Acute THA for displaced acetabular fractures in the elderly is associated with significant post-operative complications and relatively poor clinical outcomes. However, we believe there may be an indication for this treatment when there is an associated ipsilateral fracture of the femoral neck or femoral head

Porous-coated acetabular hemispherical components have proven successful in all but the most severe revision acetabular defects. A revision jumbo porous coated component has been defined as a cup with minimum diameter of 66mm in men and 62mm in women. In published studies this size cup is used in 14–39% of acetabular revisions. The advantages of this technique are ease of use, most deficiencies can be treated without structural graft, host bone contact with the porous surface is maximised, and the hip center is generally normal. Jumbo cups are typically used in Paprosky Type 2, 3A, and many 3B defects. Requirements for success include circumferential acetabular exposure, an intact posterior column, and much of the posterior wall. The cup should be stable with a press-fit between the ischium and anterior superior acetabulum with the addition of some superior lateral support. Additional support is provided with multiple dome or rim screws. Survivorship of the metal shell with revision for any reason has been reported to be 80%-96% at time frames from 15–20 years. The most common post-operative complication is dislocation

Introduction. Acetabular fractures are a challenging problem. It has been published that outcome is dependent upon the type of fracture, the reduction of the fracture and concomitant injuries. The end-points of poor outcome include avascular necrosis of the femoral head, osteoarthritis. However, we lack definitive statistics and so counselling patients on prognosis could be improved. In order to achieve this, more outcome studies from tertiary referral centres are required. We present the first long term follow up from a large tertiary referral Centre in Ireland. Methods. We identified all patients who were ten years following open reduction and internal fixation of an acetbular fracture in our centre. We invited all of these patients to attend the hospital for clinical and radiographic follow-up. As part of this, three scoring systems were completed for each patient; the Short-form 36 health survey (SF36), the Merle d'Aubigné score and the Short Musculoskeletal Functional Assessment (SMFA). Results. The data represents one years activity at a new tertiary referral unit. We idenfied a total of 44 patients who were ten years following ORIF of acetabular fractures in our unit. 21 patients (48%) replied to written invitation and attended the hospital for clinical and radiographic follow-up. A further 7 patients were contacted by telephone and interviewed to guage their rehabilitation. 3 patients had passed away. The remaining 13 patients were not contactable. Of those who attended in person for follow-up; 18 were male and 3 were female. The mean age at follow-up was 40.5 years (Range 27-60). In terms of fracture pattern epidemiology, 43% of patients sustained posterior column and wall fractures, 29% posterior wall, 14% posterior column alone, 9.5% transverse with posterior wall and 9.5% bicolumnar. 2 patients in the follow-up group had total hip replacements. Of the remaining patients the overall mean SF36 score was 78.8% (SD 16.4). The mean SMFA was 14.1% (SD 5). The mean Merle d'Aubigné score was 14.9 (SD 3.2) with 63% graded as good or excellent. Comparison of outcome between sub-groups according to fracture clasification showed no significant difference. Traumatic sciatic nerve injury was sustained by four patients in the follow-up group and all patients continued to complain of ongoing weakness. Of the patients who were contacted via telephone, 2 had total hip replacements. The remaining 5 reported no significant problems with their hips and cited this as the reason for not attending follow-up. Conclusion. Overall the outcome of the patients was more favourable than expected. This was supported by the results of the clinical scoring systems. In some patients this also appeared to be despite poor radiographic findings. Our observations suggest that concomitant injuries, especially sciatic nerve injury have a profound negative influence on the patients' ability to fully rehabilitate. These data provide a valuable tool for the trauma surgeon in providing the patient with an educated prognosis

Objective. To define the common pathology of the hips with irreducible posterior dislocation combined with femoral head fracture and the outcome of surgical treatment using surgical hip dislocation technique. Design: retrospective observational clinical study. Setting: Level III referral trauma center. Patients/Participants: from January 2011till February 2014 five patients with irreducible posterior hip dislocation and femoral head fracture who underwent operation were included and they followed for at least 18 months. Intervention. Open reduction and internal fixation of fractured femoral head and labral repair by suture anchors using surgical hip dislocation through trochanteric flip osteotomy approach. Main Outcome Measures: Clinical and radiographical findings of the irreducible posterior hip dislocation, intraoperative findings, clinical outcomes using Merle d'Aubigné & Postel and Thompson & Epstein scores, and radiological outcome. Results. All patients presented clinically with a shortened lower limb in neutral or external rotation of the hip (not in Internal rotation). All were Pipkin type II fracture of femoral head with the intact part of the head buttonholed on the posterior wall of the acetabulum through a capsule-labral flap. Postoperative computed tomography revealed perfect reduction except one case with severe comminution with good reduction. Only one patient with delayed operative management developed avascular necrosis and underwent total hip arthroplasty. Conclusion. Irreducible femoral head fracture-dislocation is rare injury with different clinical presentation that shows neutral or externally rotated limb and optimal surgical management is not clear. Surgical hip dislocation gives full access to the femoral head for reconstruction and opportunity to direct repair of the labral tears

INTRODUCTION:. Acetabular retroversion has been implicated as a risk factor for the development of early hip osteoarthritis. In clinical practice standard osseous signs such as the cross-over sign (COS) and the posterior wall sign (PWS) are widely used to establish the diagnosis of acetabular retroversion on plain radiographs. Despite standardized radiological evaluation protocols, an increased pelvic tilt can lead to a misdiagnosis of acetabular retroversion in AP radiographs and 2D MR or CT scans. Previous studies have shown that the elimination of observer bias using a standardized methodology based on 3D-CT models and the anterior pelvic plane (APP) for the assessment of COS and PWS results in greater diagnostic accuracy. Using this method a prevalence of 28% for COS and 24% for PWS has been found in a cohort of patients with symptoms indicative of FAI, however the prevalence of both signs in asymptomatic adults remains unknown. This study therefore sought to establish the prevalence of the COS and PWS in relation to the APP in an asymptomatic population using a reliable and accurate 3 D-CT based assessment. METHODS:. A large pool of consecutive CT scans of the pelvis undertaken in our department for conditions unrelated to disorders of the hip was available for analysis. Scans in subjects with a Harris hip score of less than 90 points were excluded leaving a sample of 100 asymptomatic subjects (200 hips) for this study. A previously established 3D analysis method designed to eliminate errors resulting from variations in the position and orientation of the pelvis during CT imaging was applied to determine in order to assess the prevalence of the COS and PWS in relation to the APP. Here, the acetabuli were defined as retroverted if either the COS, PWS or both were positive. RESULTS:. From the total of 200 hips a positive COS was identified in 24% (48/200) and a positive PWS was detected in 6.5% (13/200) relative to the APP using the CT data. A. In male adults a COS was observed in 25.4% (29/114) and a PWS in 10.5% (12/114). In female adults a COS were observed in 22.1% (19/86) and a PWS in 1.2% (1/86). DISCUSSION:. The high incidence of acetabular retroversion observed using an accurate 3D-CT based methodology shows that this anatomic configuration might not differ in frequency between asymptomatic individuals and patients with symptomatic FAI. Patients presenting with hip pain and evidence of FAI should therefore be subjected to strict diagnostic scrutiny, as the presence of a COS and/or PWS shows a poor correlation with the presence of symptomatic disease. In our collective of asymptomatic adults the COS showed a higher incidence than the PWS. Additionally a deficiency of the posterior acetabular wall was rare in asymptomatic adults compared to FAI patients. Therefore, the question whether an abnormal acetabular version does indeed lead to the development of osteoarthritis in all patients warrants further study. Although an association between osteoarthritis and femuro-acetabular impingement is believed to exist, long-term epidemiological studies are needed to establish the natural history of these anatomical configurations


Introduction. The ESP prosthesis is a one-piece deformable but cohesive interbody spacer. it provides 6 full degrees of freedom about the 3 axes including shock absorption (fig1). The prosthesis geometry allows limited rotation and translation with resistance to motion (elastic return property) aimed at avoiding overload of the posterior facets. The rotation center can vary freely during motion in this “silentblock” implant. It thus differs substantially from current prostheses. Material and methods. Surgeries were performed by 2 senior surgeons in 54 women and 34 men (1level in 72 cases, 2 levels in 3 cases, hybrid construct in 13 cases). Average age was 42 (SD: 7). Average BMI was 24.2kg/m2 (SD: 3,4). Clinical data and X-rays were collected at the preoperative time and at 3, 6, 12, 24, and 60 months post-op. The analysis was performed by a single observer independent from the selection of patients and from the surgical procedure. The radiological analysis at 60 months follow-up could be realized in only 76 cases because the quality of the dynamic Xrays was not sufficient in 12 patients. We measured the ROM and the location of mean center of rotation (MCR) of the implanted and adjacent levels using the Spineview® software. The MCR is considered to reflect the quality of movement of a segment; it is localized thanks its co-ordinates. X is expressed as a percentage of the length of the vertebral end plate, and Y as a percentage of the height of the posterior wall. The usual location of the MCR is in a circle, whose center is placed between 30 and 50% of the superior vertebral endplate of the vertebra below, and whose diameter is 70% of the vertebral endplate size. Results. We did not observe device-related complications. All clinical outcomes significantly improved when compared to the pre-operative status (table 1, table 2). Mean ROM was 6,3° (SD 3,2) at the implanted level, 6,7°(5,2) at the above level, 7°(4,9) at the below level. Mean x-MCR value at the implanted level was 33.1% ± 20.8 % and mean y-MCR value −12,41% ± 30.1%. Mean x- MCR value at the above adjacent level was 25,7% ± 30.1% and mean y-MCR value 16%± 39.4%. Mean x- MCR value at the below adjacent level were 31.1% ± 29.8 % and mean y-MCR value − 0,61% ± 46.8. Conclusion. The concept of the viscoelastic ESP prosthesis is different from that of the “first generation” disc implants. This study reports encouraging clinical results about pain, function, and radiological kinematic behavior especially because the MCR appears similar to those of the natural disc described in the literature. The adaptation ability is one of the main feature of this concept as we need to consider the long term evolution of the global posture and mobility after disc replacement. Purpose. Diagnosis of acetabular retroversion is essential in femoroacetabular impingement (FAI), but its assessment from radiographs is complicated by pelvic tilt and the two-dimensional nature of plain films. We performed a study to validate the diagnostic accuracy of the crossover sign (COS) and the posterior wall sign (PWS) in identifying acetabular retroversion. Method. COS and PWS were evaluated from radiographs and computed tomography (CT) scans as the standard of reference in 50 hips of subjects with symptoms of FAI. A CT-based method using 3-D models was developed to measure the COS, PWS, true acetabular version and pelvic tilt relative to the anterior pelvic plane. The new CT-based method aimed to eliminate errors resulting from variations in the position and orientation of the pelvis during imaging. Results. A low level of agreement for COS and PWS was found between radiographs and CT scans. A positive COS strongly correlated with pelvic tilt. Conclusion. These results suggest that COS and PWS determined from anteroposterior (AP) radiographs are considerably limited by pelvic tilt and inherent limitations of radiographs. Their use as the sole basis for deciding whether or not surgical intervention is indicated seems questionable. NO DISCLOSURES
