Aims. To assess the effect of age on clinical outcome and revision rates in patients who underwent total ankle arthroplasty (TAA) for end-stage ankle osteoarthritis (OA). Methods. A consecutive series of 811 ankles (789 patients) that underwent TAA between May 2003 and December 2013 were enrolled. The influence of age on clinical outcome, including the American Orthopaedic Foot and Ankle Society (AOFAS) hindfoot score, and pain according to the visual analogue scale (VAS) was assessed. In addition, the risk for revision surgery that includes soft tissue procedures, periarticular arthrodeses/osteotomies, ankle joint debridement, and/or inlay exchange (defined as minor revision), as well as the risk for revision surgery necessitating the exchange of any of the metallic components or removal of implant followed by ankle/hindfoot fusion (defined as major revision) was calculated. Results. A significant improvement in the AOFAS hindfoot score and pain relief between the preoperative assessment and the last follow-up was evident. Age had a positive effect on pain relief. The risk for a minor or major revision was 28.7 % at the mean follow-up of 5.4 years and 11.0 % at a mean follow-up of 6.9 years respectively. The hazard of revision was not affected by age. Conclusion. The clinical outcome, as well as the probability for revision surgery following TAA, is comparable between younger and older patients. The overall revision rate of the Hintegra total ankle is comparable with other three component designs. TAA should no longer be reserved for low demand elderly patients, but should also be recognized as a viable option for active patients of younger age. Introduction. Total ankle replacement as a valid treatment for end stage ankle arthritis, is gaining popularity and every year there is an increasing number of procedures. With revision rates as high as 21% at 5 years and 43% at 10 years there is a need for understanding and reporting the outcome of revision ankle replacement. Our aim was to study the patient reported outcomes following revision TAR with a minimum of 2 year follow up. Methods. All patients that underwent a revision total ankle replacement between 2012 and 2016 were included in the study. All patients received a post-operative questionnaire comprising of MOX-FQ score, EQ-5D (UK) and Foot and Ankle outcomes scores (FAOS) and patients satisfaction questionnaire with a minimum of 2 years follow up. Results. 33 patients had a revision total ankle replacement between 2012 and 2016. 2 patients were deceased therefore 31 patients were included in the study. 4 patients declined participation for completing questionnaires. We received 15/27 (55.5%) completed questionnaires. The mean MOX-FQ average domain score for pain was (50.6 ± 26.9), walking/standing (62.4 ± 36.5) and social function was (43.7± 31.0). The mean FAOS scores were (52.5 ± 30.6; pain), (54.5 ± 29.2; symptoms), (62.1 ± 30.5; ADL) and (35.5 ± 28.2; for quality of life). The mean overall health score today for EQ-5D was 73.9/100. 50% of patients were satisfied with the pain relief and return to sports and recreation obtained following the operation, 57% were satisfied with the improved in daily activities. 78.5% were overall satisfied with the results from surgery. Conclusion. Revision total ankle replacement gives overall satisfactory results demonstrated from patients reported outcomes at a minimum of 2 years following surgery

Background. Hallux rigidus is a common condition characterised by first metatarsophalangeal joint (MTPJ) degeneration, pain and limited range of motion (ROM). The gold standard surgical treatment is arthrodesis, providing good pain relief but sacrifices ROM. Recently the Cartiva synthetic cartilage implant (SCI) has been utilised as an interpositional arthroplasty, aiming to reduce pain whilst preserving range of motion. Current evidence for Cartiva SCI is largely based on a single cohort with mixed outcomes. We sought to evaluate the clinical outcomes of Cartiva SCI compared to arthrodesis undertaken in our centre. Methods. Retrospective review of patients undergoing Cartiva SCI or arthrodesis for treating hallux rigidus was conducted. Preoperative arthritis was radiographically graded using the Vanore classification. Patient reported outcomes (PROMs) were assessed using EuroQol 5-dimension score (EQ-5D-5L) and Manchester-Oxford Foot Questionnaire (MOXFQ). Results. Between 2017 and 2020 there were 33 cases (17 Cartiva, 16 arthrodesis, mean age 59.0±9.9 years) with a mean follow up of 2.3 years. For the first MTPJ arthrodesis cohort, the MOXFQ domain scores were: Index 3.9±5.8, Walking/Standing 5.1±7.6, Pain 3.2±5.0, and Social Interaction 2.6±4.0. EQ-5D-5L Index score was 0.828±0.270 and the EQ-VAS was 72.5±23.3. For the Cartiva cohort, the MOXFQ domain scores were: Index 7.7±6.0, Walking/Standing 8.9±7.9, Pain 7.1±5.0, and Social Interaction 6.4±5.4. EQ-5D-5L Index score was 0.631±0.234 and the EQ-VAS was 74.8±20.8. There was no statistically significant difference between any MOXFQ domain or EQ-5D-5L scores. However, a negative trend in MOXFQ domains was identified for the Cartiva group, as well as a reoperation rate of 23.5%. Conclusions. The Cartiva SCI demonstrated no advantage over arthrodesis in PROMs, despite the presumed benefit of preserved ROM. A significant reoperation rate was also observed. Surgeons should be cautious in the use of this novel implant

Introduction. Hallux Rigidus affects 2–10% of population, usually treated with cheilectomy or arthrodesis, however, for the subclass of patients who refuse to undergo fusion, Arthroplasty is an alternative solution, it maintain some degree of motion and provide pain relief. Toefit; is one of the prostheses being used. It is a total joint replacement with polyethylene insert. The aim of this study is to find clinical and radiological outcomes of Toefit arthroplasty. Method. A prospective study. Ethical committee approval was obtained. Patient who have received Toefit Arthroplasty with at least 12 months follow-up and were willing to participate in the study were included. Patients were reviewed by independent surgeon. Questionnaires were completed followed by clinical examination. This followed by radiographic assessment. Patients, who were willing to take part in the study but could not attend a clinical review, were invited to participate in telephone questionnaire. Pre and postoperative AOFAS scores were compared, patients' satisfaction and clinical and radiological outcome were assessed using descriptive statistics, t-test and survivalship analysis were done. Results. 180 patients had Toefit (September 2004–June 2011). 160 patients participated in the study (170 prostheses), 87% were females. Age range (38–89) year. AOFAS improved significantly from 38 to 83, with average arc of movement of 37 degrees. Patient satisfaction was high, VAS score1. Failure rate of 4.9%, there was high rate of revision of 29% due to sesamoid pain or stiffness in the initial group of patients, decreased to 8% in the second group. Radiological review showed asymptomatic aseptic loosening of 20%, mainly of the proximal phalanx components. Conclusion. First MTP joint replacement can provide pain relief and maintain good range of movement. However, this study highlighted high rate of revision and aseptic loosening. Long term review is required

Aims

A local injection may be used as an early option in the treatment of Morton’s neuroma, and can be performed using various medications. The aim of this study was to compare the effects of injections of hyaluronic acid compared with corticosteroid in the treatment of this condition.

Methods

A total of 91 patients were assessed for this trial, of whom 45 were subsequently included and randomized into two groups. One patient was lost to follow-up, leaving 22 patients (24 feet) in each group. The patients in the hyaluronic acid group were treated with three ultrasound-guided injections (one per week) of hyaluronic acid (Osteonil Plus). Those in the corticosteroid group were treated with three ultrasound-guided injections (also one per week) of triamcinolone (Triancil). The patients were evaluated before treatment and at one, three, six, and 12 months after treatment. The primary outcome measure was the visual analogue scale for pain (VAS). Secondary outcome measures included the American Orthopaedic Foot and Ankle Society (AOFAS) score, and complications.

Background. Plantar fasciitis is a frequently chronic and disabling cause of foot pain in adults. This prospective study aims to evaluate the analgesic effect of ultrasound guided dry needling in patients with chronic plantar fasciitis, refractory to conservative treatment. Method and Materials. Patients undergoing dry needling for plantar fasciitis, followed by perifascial injection between Jan ′09 and Feb ′10 were identified. Pain scores were recorded on a visual analogue scale from 0 (no pain) to 10 (worst pain possible) before the procedure, 2 weeks and 6 weeks post procedure. Patients were also contacted in April to assess their current pain level. Responses were graded as excellent (> 75% pain score improvement), good (50-74%), fair (25-49%) and poor (<25% relief). Any complications or need for any further intervention were recorded. Results. 55 feet (46 patients) were identified. All patients had had no relief with previous treatment. 24 feet who had calf tightness had undergone a gastrocnemius muscle lengthening. All patients had radiological confirmation of diagnosis prior to undergoing the procedure. At 2 weeks: 1 foot lost to follow-up. 43/54 feet had excellent or good pain relief. At 6 weeks: 5 feet (4 patients) were lost to follow up. 37 feet (28 patients) reported excellent or good pain relief. 11/13 feet patients with fair to poor response underwent a repeat procedure. Redo-Procedure. Using the same outcome measures, 8/11 and 9/11 feet had excellent or good response at 2 and 6 weeks respectively. Long-term follow-up:. 42 feet (34 patients) were contacted. 13 feet (12 patients) lost to follow-up. Follow-up duration from 2 to 13 months. Mean F/U = 7 months. 23 feet (5 redos) had excellent or good outcome 19 feet (6 redos) had fair or poor outcome. If symptoms reoccurred, average duration of total analgesic effect was 3 months. Conclusion. 80% and 74% of patients had excellent or good outcome at 2 and 6 weeks respectively. After an average of 7 months follow-up, 55% patients had excellent or good results. In those patients in whom symptoms recurred, the average duration of total analgesic effect was 3 months. We believe that ultrasound guided dry needling is an effective treatment method for plantar fasciitis

Aims

The Cartiva synthetic cartilage implant (SCI) entered mainstream use in the management of first metatarsophalangeal joint (MTPJ) arthritis following the positive results of large trials in 2016. Limited information is available on the longer-term outcomes of this implant within the literature, particularly when independent from the originator. This single-centre cohort study investigates the efficacy of the Cartiva SCI at up to five years.

Methods

First MTPJ arthritis was radiologically graded according to the Hattrup and Johnson (HJ) classification. Preoperative and sequential postoperative patient-reported outcome measures (PROMs) were evaluated using the Manchester-Oxford Foot Questionnaire (MOXFQ), and the activities of daily living (ADL) sub-section of the Foot and Ankle Ability Measure (FAAM).

Aims

The optimal management of posterior malleolar ankle fractures, a prevalent type of ankle trauma, is essential for improved prognosis. However, there remains a debate over the most effective surgical approach, particularly between screw and plate fixation methods. This study aims to investigate the differences in outcomes associated with these fixation techniques.

Methods

We conducted a comprehensive review of clinical trials comparing anteroposterior (A-P) screws, posteroanterior (P-A) screws, and plate fixation. Two investigators validated the data sourced from multiple databases (MEDLINE, EMBASE, and Web of Science). Following PRISMA guidelines, we carried out a network meta-analysis (NMA) using visual analogue scale and American Orthopaedic Foot and Ankle Score (AOFAS) as primary outcomes. Secondary outcomes included range of motion limitations, radiological outcomes, and complication rates.

Aims. Few reports compare the contribution of the talonavicular articulation to overall range of movement in the sagittal plane after total ankle arthroplasty (TAA) and tibiotalar arthrodesis. The purpose of this study was to assess changes in ROM and functional outcomes following tibiotalar arthrodesis and TAA. Patients and Methods. Patients who underwent isolated tibiotalar arthrodesis or TAA with greater than two-year follow-up were enrolled in the study. Overall arc of movement and talonavicular movement in the sagittal plane were assessed with weight-bearing lateral maximum dorsiflexion and plantarflexion radiographs. All patients completed Short Form-12 version 2.0 questionnaires, visual analogue scale for pain (VAS) scores, and the Foot and Ankle Ability Measure (FAAM). Results. In all, 41 patients who underwent TAA and 27 patients who underwent tibiotalar arthrodesis were enrolled in the study. The mean total arc of movement was 34.2° (17.0° to 59.1°) with an average contribution from the talonavicular joint of 10.5° (1.2° to 28.8°) in the TAA cohort. The average total arc of movement was 24.3° (6.9° to 44.3°) with a mean contribution from the talonavicular joint of 22.8° (5.6° to 41.4°) in the arthrodesis cohort. A statistically significant difference was detected for both total sagittal plane movement (p = 0.00025), and for talonavicular motion (p < 0.0001). A statistically significant lower VAS score (p = 0.0096) and higher FAAM (p = 0.01, p = 0.019, respectively) was also detected in the TAA group. Conclusion. TAA preserves more anatomical movement, has better pain relief and better patient-perceived post-operative function compared with patients undergoing fusion. The relative increase of talonavicular movement in fusion patients may play a role in the outcomes compared with TAA and may predispose these patients to degenerative changes over time. Take home message: TAA preserves more anatomic sagittal plane motion and provides greater pain relief and better patient-perceived outcomes compared with ankle arthrodesis. Patients with advanced stage hallux rigidus from 12 centers in Canada and the UK were randomized (2:1) to treatment with a small (8/10 mm) hydrogel implant (Cartiva) or 1. st. MTP arthrodesis. VAS pain scale, validated outcome measures (FAAM sport scale), great toe active dorsiflexion motion, secondary procedures, radiographic assessment and safety parameters were evaluated. 236 patients were initially enrolled, 17 patients withdrew prior to randomization, 17 patients withdrew after randomization and 22 were non-randomized training patients, leaving 152 implant and 50 arthrodesis patients. Standard demographics and baseline outcomes were similar for both groups. Mean VAS pain scores decreased from 6.8 and 6.9 respectively for the implant and arthrodesis groups from baseline to 1.4 and 0.7 at 24 months. Similarly, the FAAM sports score improved significantly from baseline levels of 37 and 36 to 24 months level of 77 and 82 respectively for the implant and arthrodesis groups. First MTP active dorsiflexion motion improved an average of 4° at 3 months after implant placement and was maintained at 24 months. Secondary surgeries occurred in 17 (11.2%) implant patients and 6 (12.0%) arthrodesis patients. Fourteen (9.2%) implants were removed and converted to arthrodesis and 6 (12.0%) arthrodesis patients had painful hardware requiring removal. There was no case of implant fragmentation, wear, or bone loss. Analysis of a single composite endpoint utilizing the three primary study outcomes (pain, function, and safety) showed statistical equivalence between the2groups. Conclusion. In patients requiring surgery for advanced stage hallux rigidus, treatment with a small synthetic cartilage implant resulted in comparable clinically important pain relief and functional outcomes compared to 1. st. MTP arthrodesis while preserving and often improving great toe motion. Secondary surgical intervention was similar in the implant and arthrodesis groups. Revision from a small implant plug to arthrodesis can be performed if needed

Introduction:. Ankle arthritis is a leading cause of pain and disability. The effect of this condition on physical and mental health is similar to end stage hip arthritis. There is paucity of literature on PROMS following total ankle replacements (TAR) in comparison to total hip replacement (THR) or knee replacement (TKR). We aimed to study 5 year outcomes of TAR in comparison with TKR and THR. Methods:. PROMS data from patients who underwent a primary THR, TKR or TAR from March 2003 to 2013 were collected from our hospital patient registry. They were divided into 3 groups based on the type of primary joint replacement. Patient demographics and patient reported outcomes (WOMAC, SF-36 scores and patient satisfaction scores at follow up) were compared at pre-op and 5 year follow up. Results:. There was data available on 1920 THR, 2582 TKR and 248 TAR patients. Pre-operatively, TAR patients reported higher function scores when compared to THR and TKR (40.2 vs. 34.2 and 35.8; p<0.05). For SF-36 scores, there was no difference between groups for general health, role emotional components (P>0.05); TAR patients reported similar scores to TKRs for physical domains; to THRs for the mental domains (P>0.05). At 5 years post-op, TARs reported lower scores than THRs and TKRs for function and stiffness. For SF-36 scores, TARs reported similar outcomes to THR and TKR for mental health components (p>0.05), similar scores to TKR for 3/4 physical domains (p<0.05), but lower satisfaction rates for ADL and recreation when compared to THR (P<0.05). Conclusion:. TAR patients had similar outcomes to THR or TKR patients for disease specific and mental health domains, and lower patient satisfaction rates in terms of pain relief, ADL and recreation. Further research is warranted including clinical outcomes along with PROMS with a long term follow up

It has been suggested that extracorporeal shockwave therapy is a safe and effective treatment for pain relief from recalcitrant plantar fasciopathy (PF). However, the changes in gait and associated biomechanical parameters have not been well characterised. We recruited 12 female patients with recalcitrant PF who had a mean age of 59 years (50 to 70) and mean body mass index of 25 kg/m. 2. (22 to 30). The patients reported a mean duration of symptoms of 9.3 months (6 to 15). Shockwave therapy consisting of 1500 impulses (energy flux density 0.26 mJ/mm. 2. ) was applied for three sessions, each three weeks apart. A pain visual analogue scale (VAS) rating, plantar pressure assessment and motion analysis were carried out before and nine weeks after first shock wave therapy. It was demonstrated that patients increased their walking velocity and cadence as well indicating a decrease in pain after shockwave therapy. In the symptomatic foot, the peak contact pressure over the forefoot increased and the contact area over the digits decreased. The total foot impulse also decreased as did stance duration. The duration the centre of pressure remained in the hindfoot increased in the symptomatic foot after shockwave therapy. The differences in centre of pressure trajectory at baseline decreased at final follow-up. In conclusion, shockwave therapy not only decreased the pain VAS rating but also improved the gait parameters of the symptomatic foot in PF patients. Achieving arthrodesis of the ankle can be difficult in the presence of infection, deformity, poor soft tissues and bone loss. We present a series of 48 patients with complex ankle pathology, treated with the Ilizarov technique. Infection was present in 30 patients and 30 had significant deformity before surgery. Outcome was assessed clinically and with patient-reported outcome measures (Modified American Orthopaedic Foot and Ankle Society (MAOFAS) scale and the Short-Form (SF-36)). Arthrodesis was achieved in 40 patients with the Ilizarov technique alone and in six further patients with additional surgery. Infection was eradicated in all patients at a mean follow-up of 46.6 months (13 to 162). Successful arthrodesis was less likely in those with comorbidities and in tibiocalcaneal fusion compared with tibiotalar fusion. These patients had poor general health scores compared with the normal population before surgery. The mean MAOFAS score improved significantly from 24.3 (0 to 90) pre-operatively to 56.2 (30 to 90) post-operatively, but there was only a modest improvement in general health; the mean SF-36 improved from 44.8 (19 to 66) to 50.1 (21 to 76). There was a major benefit in terms of pain relief. Arthrodesis using the Ilizarov technique is an effective treatment for complex ankle pathology, with good clinical outcomes and eradication of infection. Introduction:. In a consecutive series of 71 arthroscopic subtalar arthrodeses performed between 2004 and 2011, 14 also involved arthroscopic decortication of the talonavicular joint (double arthrodesis) and 4 the subtalar, talonavicular and calcaneocuboid joints (triple arthrodeses). Methods:. We examined complications, union rates in all 18 patients and clinical outcomes in 16 for whom this was the sole procedure. Results:. Mean age was 62 (45–78). Two talonavicular joints failed to unite and a third patient suffered a diabetic Charcot midfoot neuro-arthropathy. These patients' outcomes were classified as poor. Two patients underwent planned major ankle or midfoot surgery in addition to arthroscopic double arthrodeses. These joints united but these patients were not included in the clinical review to avoid confounding outcomes. Mean follow-up for the remaining 13 patients was 4.4 (1.75–7.5) years. There were no immediate perioperative complications. All 4 patients with triple fusions united with good or excellent outcomes. The nine patients receiving double arthrodesis united with 8 good or excellent outcomes. The remaining patient reported good deformity correction and stability but disappointing pain relief, (classification poor). Conclusions:. Double and triple arthrodeses remain valid salvage options for painful arthrosis and severe deformity. Preservation of the calcaneocuboid joint permits a relative lateral column lengthening when correcting planovalgus deformity. Arthroscopic surgery offers preservation and protection of soft tissues and reduces wound tension. The sinus tarsi approach permits good visualisation and decortication of the triple joints and rotatory correction of deformity. This technique is not appropriate when there is extensive bone loss requiring block bone grafting. Early complications are reduced and late complications such as non-union and arthrosis of adjacent joints seem similar to those reported in studies on open arthrodeses

Introduction. Excision of prominent metatarsal heads for severe rheumatoid forefoot deformity is well established in clinical practice but results may deteriorate with time. The Stainsby forefoot arthroplasty however, recognises the pathological anatomy of the deformity and is designed to preserve the metatarsal heads by repositioning the plantar plates and forefoot fat pad underneath them. Design. A prospective case series observing the clinical outcomes and changes in pedobarograph patterns in rheumatoid arthritis patients with severe deformity undergoing Stainsby forefoot arthroplasty. Materials and methods. Twelve patients (21 operated feet) were reviewed at 5 years. AOFAS scores and pedobarographs were recorded pre-operatively and at 5 years post-operatively. Results. AOFAS scores improved significantly from 21 ± 15 pre-operatively to 61 ± 12 at 5 years (p<0.0001) with most of the improvement occurring in the pain score. Pre-operative pedobarographs showed a concentration of forefoot loading, either under the 1st metatarsal or metatarsals 2 and 3, in 38% of patients. Post-operatively there was a significant reduction of peak loading under the 1st metatarsal and metatarsals 2 and 3 and a more even distribution of loading under the forefoot. The duration of the forefoot stance phase as a percentage of total stance phase improved from 24% pre-operatively to 31% post-operatively. Conclusions. Patient reported outcomes indicate the Stainsby procedure provides increased function and lasting pain relief. This is further supported by pedobarograph data showing improvement in the pressure distribution and stance times during gait. We therefore recommend this procedure for management of severe rheumatoid forefoot deformity

Introduction. Bone marrow oedema syndrome (BMES) of the foot and ankle is an uncommon and often misdiagnosed condition. It is usually thought to be a benign self limiting condition, without any sequelae. However, it can cause disabling pain for a prolonged period of time. Materials and Methods. We retrospectively reviewed 8 patients with the diagnosis of BMES. There were 6 males and 2 females with an average age of 51 years (38–63 years). All patients had acute onset of severe pain in the foot without any history of trauma. None of the patients had history of excessive alcohol or steroid intake. 5 patients (63%) had bilateral involvement of migratory nature. All patients had characteristic features on MR scans, with involvement of 3 or more bones in the foot. Results. All patients had their limbs immobilised in a below knee cast and asked to mobilise non weight bearing for 6–8 weeks. Thereafter, they were allowed to mobilise with a pneumatic walker, increasing weight bearing as pain allowed. The walker was discarded at an average of 26 weeks. All 8 patients had complete relief of pain after an average of 14 months (4–22 months). All were able to return to their work environment but 2 complained of occasional aching and swelling of their feet, at the end of their shift at work. 2 patients were treated with bisphosphonates and 2 had targeted local anaesthetic injections with good pain relief. None were treated with Iloprost. Conclusions. BMES affecting the foot and ankle is an uncommon condition. Symptoms tend to be prolonged and may result in chronic swelling and aching around the ankle. Symptoms do tend to improve with time with or without intervention

Aim. We aimed to study the effect of BMI on clinical and patient-reported outcomes in patients with TAR with a minimum follow-up of three years. Method. Patients who underwent a TAR between March 2006 and May 2009 were invited to take part in the hospital patient registry. Patients were divided into two groups based on BMI (Group A – BMI <30 and Group B – BMI >30). Patient demographics, co-morbidities, clinical (AOFAS), patient reported outcomes (FAOS, SF-36, patient satisfaction) and complications were collected pre-operatively and at 1, 2 and 3 years and comparison made between groups. Results. There were 50 patients in group A and 31 in group B. There was no significant difference between age (Mean 64.5 (A) and Mean 61.3 (B), gender and side of operation between the groups (p>0.05). Group A had higher percentage of patients with OA and RA compared to Group B (p=0.027). Group B (1.16) reported higher co-morbidites than Group A (1.81); p=0.012. There was no difference in AOFAS scores, FAOS scores (for pain, function and stiffness) and SF-36 scores reported between groups at 1, 2 and 3 years post-operatively (p>0.05). At 3 years, Group B reported less patient satisfaction with return to ADL 84.6 %(A) vs. 50.1%(B) and recreational activities 73.1 %(A) vs. 43.8 %(B); p<0.05. There was no significant difference in overall satisfaction and pain relief and also in the number of reported complications between Groups A and B (p>0.05). Conclusion. Patients with BMI > 30 reported higher co-morbidities pre-operatively and less patient satisfaction (return to ADL and recreation) at 3 years when compared to patients with BMI <30. There was no difference in clinical, complications and patient reported outcomes between these groups

Background. Postoperative pain following (Mobility TM) ankle arthroplasty (AA) is recognised problem. This study aimed to determine pattern of postoperative pain following Ankle arthroplasty (AA). Materials and Methods. In prospective observational study 135 patients who had (AA) and follow-up of 12–36 months were included. AOFAS ankle score, patients' satisfaction, SF36 and diagrammatic mapping of postoperative pain among other parameters were collected preoperatively and postoperatively at 3 months, 6 months and the annually. Patients with AOFAS of < 50 with postoperative ankle pain were examined in details. Results. From total of 135 of patients with follow-up of 12 months, (12.5%)17 patients have low AOFAS score and ankle pain, 11(12.5%) of 85 patients with 2 year follow-up and (10.6%),5 of 47 patients with 3 year follow-up. Most of patients with low AOFAS score during first year have improved but 3 patients. Different group patients developed ankle pain during the second and third year. 3 patients of 5, in year 3 follow-up have medial side ankle pain and 2 lateral; similarly there is more medial sided pain during year 2 review (7/11). There is more medial sided pain noticed during first postoperative year as well. Lateral pain seems to relate with subtalar joints problems. Medial side pain is less understandable, it might be due to tension in medial ankle ligaments. Our study showed improvement in AOFAS score and pain relief associated with medial ligaments release or medial malleolus fracture. Conclusion. There are 10–13 % of low AOFAS scores following Ankle Arthroplasty with ankle pain. There are emerging evidences explaining postoperative ankle pain. Further studies are required in this field

Data is scant on the critical question of whether patients with endstage ankle arthritis are better served by a fusion or a replacement. The STAR trial, a prospective case control study, comparing safety and efficacy of STAR ankle replacement at 24 months for 158 replacements and 66 fusions:. This FDA trial showed the STAR ankle replacement had better function, equivalent pain relief and a higher rate of complications and secondary procedures as ankles treated with fusion. A separate prospective cohort comparison of 200 ankle replacements vs. 94 ankle fusions performed by the collaborative consortium of Canadian Orthopaedic Foot and Ankle Surgeons (COFAS) suggests similar patient oriented outcomes at 24 months. Patients' self-assessment questionnaires do not show significant differences between the two groups. The main medium/long-term concerns with ankle replacement remain component subsidence (especially talar subsidence) and polyethylene wear. Forces across the ankle are considerable, and the orientation of those forces to the underlying trabecular structure are a concern. Whether 2 or 3 part ankles will provide better bearing wear results remains unknown. With older designs, at 10 years the Swedish registry found approximately 60% survivorship. This registry has also shown better survivorship in patients with rheumatoid disease and with increased surgeon experience. More recent results related to the Hintegra and AES components show better midterm (5 year) revision rates in selected surgeons hands, than seen with older designs. A large meta-analysis of ankle fusion and replacement published data by Haddad et al reported a mean nonunion rate of 10%. The main long-term concerns with ankle fusion are limitations of motion, and the development of premature arthritis in adjacent foot joints. In a study of an average of 9 year outcome of the Agility ankle replacement suggested that the incidence secondary arthritis is halved by replacement as compared to fusion

Introduction. Many common fractures are inherently stable, will not displace and do not require plaster casting to achieve union in a good position. Nevertheless, many patients with stable fractures are advised that they need a cast, despite the potential for stiffness, skin problems and thromboembolism. Attempts to challenge this practice often meet the argument that patients prefer a cast for pain relief. We analysed five years of a single consultant's fracture clinic to see how many patients with stable foot and ankle fractures chose a cast after evidence-based counselling. Materials and methods. All patients with stable fractures of the ankle or metatarsals seen between 1st June 2005 and 31st May 2010 were included. Displaced or potentially unstable ankle fractures, Jones fractures and fractures involving the Lisfranc joint were excluded. Patients were advised functional treatment but offered a cast if they wished. Patients were documented prospectively as part of a larger audit, including demographics, diagnosis and treatment in the emergency department and fracture clinic. Results. 93 patients had stable ankle fractures. One (1%) chose a cast, 77 an ankle brace and 14 the RICE regime. One was advised a cast for neurological deformity. 105 patients had fifth metatarsal fractures outside the “non-” zone. 19 (18%) chose casts and 86 followed the RICE regime. 50 patients had other stable metatarsal fractures 15 (30%) chose casts. 86% of patients had casts applied in the emergency department. Discussion. The majority of patients with stable foot and ankle fractures do not wish to wear a cast once they understand it will not affect their outcome. In many cases this decision could have been reached in the emergency department with appropriate guidelines and education, preventing patient inconvenience and possible adverse events. Conclusion. Most patients are happy with evidence-based functional treatment of stable fractures
