Introduction. Developmental dysplasia of the hip (DDH) can be managed through a variety of different surgical approaches from closed reduction to simple tenotomies of the adductors and through to osteotomies of the femur and pelvis. The rate of redislocation following open reduction for the treatment of DDH may be affected by the number of intraoperative surgeons. Materials and methods. We performed a retrospective cohort analysis of 109 patients who underwent open reduction with or without bony osteotomies as a primary intervention between 2013 and 2023. We measured the number of redislocations and number of operating surgeons (either 1 or 2 operating surgeons) to assess for any correlation. 109 patients were identified and corresponded to 121 primary hip operations, the mean age at operation was 82.2 months (range 6 to 739 months). During the 10-year period 7 hip redislocations were identified. Results. Of the 7 redislocated hips, the rate of redislocation was found to be higher in patients who had undergone surgery via a single surgeon (5 redislocations) compared to the dual surgeon cohort (2 redislocations), though this did not reach statistical significance. Redislocation was more common in female patients and right laterality 7.2% and 8.7% respectively, though this again did not reach statistical significance. Conclusions. We conclude that a single surgeon approach, female gender and right laterality are potential risk factors for redislocation following open reduction. Further investigation utilising a larger sample size would be required to appropriately explore these potential risk factors further

Introduction. Ankle fractures in the elderly have been increasing with an ageing but active population and bring with them specific challenges. Medical co-morbidities, a poor soft tissue envelope and a requirement for early mobilisation to prevent morbidity and mortality, all create potential pitfalls to successful treatment. As a result, different techniques have been employed to try and improve outcomes. Total contact casting, both standard and enhanced open reduction internal fixation, external fixation and most recently tibiotalocalcaneal (TTC) nailing have all been proposed as suitable treatment modalities. Over the past five years popular literature has begun to herald TTC nailing as an appropriate and contemporary solution to the complex problem of high-risk ankle fragility fractures. We sought to assess whether, within our patient cohort, the outcomes seen supported the statement that TTC has equal outcomes to more traditional open reduction internal fixation (ORIF) when used to treat the high-risk ankle fragility fracture. Materials & Methods. Results of ORIF versus TTC nailing without joint preparation for treatment of fragility ankle fractures were evaluated via retrospective cohort study of 64 patients with high-risk fragility ankle fractures without our trauma centre. We aimed to assess whether results within our unit were equal to those seen within other published studies. Patients were matched 1:1 based on gender, age, Charlson Comorbidity Index (CCI) and ASA score. Patient demographics, AO/OTA fracture classification, intra-operative and post-operative complications, discharge destination, union rates, FADI scores and patient mobility were recorded. Results. There were 32 patients within each arm. Mean age was 78.4 (TTC) and 78.3 (ORIF). The CCI was 5.9 in each group respectively with mean ASA 2.9 (TTC) and 2.8 (ORIF). There were two open fractures within each group. Median follow up duration was 26 months. Time to theatre from injury was 8.0 days (TTC) versus 3.3 days (ORIF). There was no statistically significant difference in 30-day, one year or overall mortality at final follow up. Kaplan-Meier survivorship analysis did however demonstrate that of those patients who died post-operatively the mean time to mortality was significantly shorter in those treated with TTC nailing versus ORIF (20.3 months versus 38.2 months, p=0.013). There was no statistical difference in the overall complication rate between the two groups (46.9% versus 25%, p=0.12). The re-operation rate was twice as high in patients treated with TTC nailing however this was not statistically significant. There was no statistical difference in the FADI scores at final follow up, 72.1±12.9 (TTC) versus 67.9±13.9 (ORIF) nor post-operative mobility status. Conclusions. Within our study TTC nailing with an unprepared joint demonstrated broadly equivalent results to ORIF in the management of high-risk ankle fragility fractures; this replicates findings of previous studies. We did however observe that mean survival was significantly shorter in the TTC group than those treated with ORIF. We believe this may have been contributed to by a delay to theatre due to TTC stabilisation being treated as a sub-specialist operation in our unit at the time. We propose that both TTC and ORIF are satisfactory techniques to stabilise the frail ankle fracture however, similarly to the other fragility fractures, the priority should be on an emergent operation in a timely fashion in order to minimise the associated morbidity and mortality. Further randomised control studies are needed within the area to establish definitive results and a working consensus

The risk of AVN is high in Unstable Slipped Capital Femoral Epiphysis (SCFE) and the optimal surgical treatment remains controversial. Our AVN rates in severe, unstable SCFE remained unchanged following the introduction of the Modified Dunn Procedure (MDP) and as a result, our practice evolved towards performing an Anterior Open Reduction and Decompression (AOR) in an attempt to potentially reduce the “second hit” phenomenon that may contribute. The aim of this study was to determine the early surgical outcomes in Unstable SCFE following AOR compared to the MDP. All moderate to severe, Loder unstable SCFEs between 2008 and 2022 undergoing either an AOR or MDP were included. AVN was defined as a non-viable post-operative SPECT-CT scan. Eighteen patients who underwent AOR and 100 who underwent MPD were included. There was no significant difference in severity (mean PSA 64 vs 66 degrees, p = 0.641), or delay to surgery (p = 0.973) between each group. There was no significant difference in the AVN rate at 27.8% compared to 24% in the AOR and MDP groups respectively (p = 0.732). The mean operative time in the AOR group was 24 minutes less, however this was not statistically significant (p = 0.084). The post-reduction PSA was 26 degrees (range, 13–39) in the AOR group and 9 degrees (range, -7 to 29) in the MDP group (p<0.001). Intra-operative femoral head monitoring had a lower positive predictive value in the AOR group (71% compared to 90%). Preliminary results suggest the AVN rate is not significantly different following AOR. There is less of an associated learning curve with the AOR, but as anticipated, a less anatomical reduction was achieved in this group. We still feel that there is a role for the MDP in unstable slips with a larger remodelling component

Management of acetabular fractures in the elderly population remains somewhat controversial in regards to when to consider is open reduction internal fixation (ORIF) versus acute primary total hip. study aims to (1) describe outcome of this complex problem and investigate predictors of successful outcome. This retrospective study analyzes all acetabular fractures in patients over the age of 60, managed by ORIF at a tertiary trauma care centre between 2007 and 2018 with a minimum follow up of one year. Of the 117 patients reviewed, 85 patients undergoing ORIF for treatment of their acetabular fracture were included in the analysis. The remainder were excluded based management option including acute ORIF with THA (n=10), two-stage ORIF (n=2), external fixator only (n=1), acute THA (n=1), and conservative management (n=1). The remainder were excluded based on inaccessible medical records (n=6), mislabelled diagnosis (n=6), associated femoral injuries (n=4), acetabular fracture following hemiarthroplasty (n=1). The mean age of the cohort is 70±7 years old with 74% (n=62) of patients being male. Data collected included: demographics, mechanism of injury, Charlson Comorbidity Index (CCI), ASA Grade, smoking status and reoperations. Pre-Operative Radiographs were analyzed to determine the Judet and Letournel fracture pattern, presence of comminution and posterior wall marginal impaction. Postoperative radiographs were used to determine Matta Grade of Reduction. Outcome measures included morbidity-, mortality- rates, joint survival, radiographic evidence of osteoarthritis and patient reported outcome measures (PROMs) using the Oxford Hip Score (OHS) at follow-up. A poor outcome in ORIF was defined as one of the following: 1) conversion to THA or 2) the presence of radiographic OA, combined with an OHS less than 34 (findings consistent with a hip that would benefit from a hip replacement). The data was analyzed step-wise to create a regression model predictive of outcome following ORIF. Following ORIF, 31% (n=26) of the cohort had anatomic reduction, while 64% (n=54) had imperfect or poor reduction. 4 patients did not have adequate postoperative radiographs to assess the reduction. 31 of 84 patients undergoing ORIF had a complication of which 22.6% (n=19) required reoperation. The most common reason being conversion to THA (n=14), which occurred an average of 1.6±1.9 years post-ORIF. The remainder required reoperation for infection (n=5). Including those converted to THA, 43% (n=36) developed radiographic OA following acetabular fracture management. The mean OHS in patients undergoing ORIF was 36 ± 10; 13(16%) had an OHS less than 34. The results of the logistic regression demonstrate that Matta grade of reduction (p=0.017), to be predictive of a poor outcome in acetabular fracture management. With non-anatomic alignment following fixation, patients had a 3 times greater risk of a poor outcome. No other variables were found to be predictive of ORIF outcome. The ability to achieve anatomic reduction of fracture fragments as determined by the Matta grade, is predictive of the ability to retain the native hip with acceptable outcome following acetabular fracture in the elderly. Further research must be conducted to determine predictors of adequate reduction in order to identify candidates for ORIF

Purpose of the study. This study aims to evaluate the use of closed reduction of hips with developmental dysplasia of the hip (DDH) and medial open reduction of these hips as a subsection of closed reduced hips. Methods. The study was a retrospective analysis of treatment of 30 children with developmental dysplasia of the hip (DDH). These children were taken from a consecutive series of children treated over a period from June 2000 to 2011 with closed reduction by a single surgeon. The ages at the time of diagnosis were between 1 day and 13 months (mean 5.25 weeks). Included in this series are 7 patients treated with medial open reduction, all done with the Ludloff approach. Follow up of these patients was from 8 months to 12 years (mean 5 years). All patients needing secondary procedures were noted. The X- rays were evaluated for percentage acetabulum cover in patients over the age of 8 and improvement of the acetabular index in all these patients. Results. 4 children needed secondary procedures. 1 child of the closed reduction group developed avascular necrosis of the femoral head that was treated with a Salter osteotomy and a further 2 needed secondary open reductions after redislocation following initial closed reduction. One child with bilateral open medial reductions had a Salter osteotomy 6 years after the initial treatment was done. 26 of the children had good outcomes with improvement of the acetabular angles, percentage acetabular cover and pain free independent ambulation. The average acetabular index improved from 37.5° to 23.3°. Conclusion. Closed reduction of DDH hips is a good treatment modality. Early treatment allows for acetabular and femoral development. There are minimal secondary procedures necessary after closed reduction, and open medial reduction does not increase the complication rate. NO DISCLOSURES

Avascular necrosis (AVN) of the femoral head is a potentially devastating complication of treatment for developmental dysplasia of the hip (DDH). AVN most commonly occurs following operative management by closed (CR) or open reduction (OR). This occurrence has frequently been examined in single centre, retrospective studies, however, little high-level evidence exists to provide insight on potential risk factors. The purpose of this observational, prospective multi-centre study was to identify predictors of AVN following operatively-managed DDH. A multi-centre, prospective database of infants diagnosed with DDH from 0–18 months was analyzed for patients treated by CR and/or OR. At minimum one year follow-up, the incidence of AVN (Salter criteria) was determined from AP pelvis radiographs via blinded assessment and consensus discussion between three senior paediatric orthopaedic surgeons. Patient demographics, clinical exam findings and radiographic data were assessed for potential predictors of AVN. A total of 139 hips in 125 patients (102 female, 23 male) underwent CR/OR at a median age of 10.4 months (range 0.7–27.9). AVN was identified in 37 cases (26.6% incidence) at a median 23 months post-surgery. Univariate logistic regression analysis comparing AVN and no AVN groups identified sex, age at diagnosis, age at surgery, pre-surgery IHDI grade and time between diagnosis and surgery as potential predictive factors. Specifically, male sex (OR 2.21 [0.87,5.72]), IHDI grade IV, and older age at diagnosis (7.4 vs. 9.5 months) and surgery (10.2 vs. 13.6 months) were associated with development of AVN. Likewise, increased time between diagnosis and surgery (2.9 vs. 5.5 months) was also associated with a higher incidence. No association was found with surgery type (CR vs. OR), pre-surgery acetabular index or surgical hip. Development of AVN occurred in 26.6% of hips undergoing CR or OR at a median 23 months post-surgery. Male sex, older age at diagnosis and surgery, dislocation severity and increased time between diagnosis and surgery were associated with AVN. Longer-term follow-up and larger numbers will be required to confirm these findings. Early outcomes from this prospective patient cohort suggest that AVN is an important complication of operative management for DDH, and appears to occur at a comparable rate whether the reduction is performed open or closed. Male patients may be more susceptible to developing AVN and merits further exploration. Potential predictive factors of older age and length of time between diagnosis and surgery emphasize the importance of early detection and treatment to minimize complications and optimize outcomes

Background. Outcome of Type II and III (Sander's CT classification) fracture of the calcaneum who underwent open reduction and internal fixation was assessed. Methods. Thirty-three type II and III fractures of the calcaneum (all unilateral) underwent open reduction and internal fixation using the ‘extensile lateral approach’. There were twelve Type II and twenty-one Type III fractures. Patients were followed up for a mean of 40.81 months (Range 28 to 62 months). Patients were assessed clinically by the Creighton Nebraska Health foundation score for assessment of fractures of the calcaneum. Radiologically assessment was done comparing the pre and post operativeBohler's and Gissane angles and measuring the calcaneal width on the axial xrays. Results. Nineteen patient were classified as excellent and 14 as good. There were no poor or fair results. The mean correction of Bohler's angle was 5.6 +/− 7.2 and the Gissane'sanglecorrected by 7.8 +/−5.8 °. Superficial skin necrosis was the most common complication encountered (3 patients). Two patients preferred removal of the implant after fracture union due to prominent hardware. Conclusion. Open reduction Type II and III intra – articular calcaneal fractures is associated with minimal complications and gives reproducible results, especially in young adults

Introduction. Lateral humeral condyle fractures account for 17% of the distal humeral condyle fractures. They affect children between 5 and 10 years of age. Recent reports advocate closed reduction and internal fixation for the less displaced fractures. Methods. We retrospectively reviewed children treated with open reduction internal fixation of these fractures at a single institution over a period of 13 years. All cases of lateral humeral condyle fractures treated with ORIF were identified through the trauma register. Case notes and radiographs were retrieved. Fracture classification, mode of fixation, time to union, and clinical examination at latest follow up were reviewed. Results. 105 lateral condyle fractures were identified. 76 (72%) in boys and 29 (28%) in girls. Average age was 6.2 years. 92 were Milch type 2 and 13 Milch type 1. According to the Jacobs classification for displacement 9 were type I, 33 Type II and 63 type III. All fractures were treated with open reduction and fixation with K-wires. Average time to union (radiologic) was 34 days. Mean follow up was 39 months. Hypertrophy of the lateral condyle in follow up radiographs was documented in 45 cases (42%). Cubitus Varus was documented in 7 cases (7%). None of the children complained of painful range of movement at latest follow up. There was one case of superficial infection of the K-wires and one case of delayed union. Discussion. Open reduction internal fixation is the established management of lateral humeral condyle fractures. This series confirms the consistently good results of open reduction. Compared to recent reports of closed reduction internal fixation, this series demonstrates good results with no complications directly relating to the open reduction technique

The management of developmental dysplasia of the hip (DDH) requiring open reduction between 12 and 18 months of age is controversial. We compare the outcome of medial approach open reduction (MAOR) versus delayed anterior open reduction with Salter osteotomy in such patients. 17 consecutive patients who underwent MAOR aged 12–20 months were reviewed (mean follow-up of 40 months, range 6–74). This group was compared to 15 controls who underwent anterior reduction and Salter osteotomy aged 18–23 months (mean follow-up of 44 months, range 14–134). 13 of the 17 (76%) MAOR patients required subsequent Salter osteotomy at a mean of 22 months post-reduction, with a further 2 patients under follow-up being likely to require one. Acetabular index improved from 42 (32–50, SD − 5.5) to 16 (7–24, SD − 4.5) in the MOAR group after Salter osteotomy compared to an improvement of 40 (30–53, SD − 6) to 13 (4–24, SD − 5) in the control group (p>0.05). Acetabular index at last follow-up was within normal limits in 15 of 17 (88%) MAOR patients. All patients in the control group had acetabular indices (or centre-edge angles of Wiberg) within the normal range. There was 1 subluxation (7%) in the control group. There were 6 cases (33%) of post-operative avascular necrosis (5 Kalamchi & MacEwen Grade I, 1 Grade 2) in the MAOR group and 6 (40%) in the control group (5 Grade 1, 1 Grade 4). All of the MAOR patients had good or excellent clinical results according to McKay's criteria, compared to 14 out of 15 (93%) controls. This study suggests that MAOR or delayed open reduction and Salter osteotomy is a reasonable treatment for children with DDH presenting between the ages of 12 and 18 months. However, the majority of MAORs are likely to require a subsequent Salter osteotomy

Objective. The objective was to assess whether late presentation of DDH leads to an increase in treatment magnitude and cost. Methods. This was a retrospective review of prospectively collected data from our hip instability clinic database. All patients presenting to our hip instability clinic that required any form of treatment for DDH between 1990 and 2005 were included. Children were grouped according to age at presentation and then treatment requirements were reviewed. Average costs were calculated based on procedures performed. Results. 84% of children presenting before 6 weeks were treated successfully with abduction bracing, versus none after the age of 10 months. The need for open reduction increased from 8% if presenting before 6 weeks to 86% for those over 10 months. This equates to a 12-fold increase in relative risk of requiring open reduction surgery. Increasing age at presentation was associated with an increase in the number of procedures required, increased magnitude of procedure and increased financial cost per patient. Conclusions. The loss of repeated screening for DDH will lead to an increase in late presentations. This work has demonstrated that increased age of presentation leads to a concomitant increase in open reduction and other operative procedures. Implementation of an additional opportunistic mandatory screening examination at 3–5 months could help to reduce the unintended effects of the new guidelines

Purpose. Open reduction and plate fixation of displaced mid-shaft clavicle fractures has gained significant popularity following a recent multi-center randomized control trial. The purpose of this study is to describe the incidence of reoperation following plate fixation of displaced mid-shaft clavicle fractures. The secondary objective is to determine if plate design influences the incidence of reoperation. Method. A retrospective search of our hospital database was performed to identify subjects treated with plate fixation for a displaced clavicle fracture between 2001 and 2009. Radiographs and medical records were used to identify demographic data, fracture classification, plate design, and reoperation events. Only mid-shaft (AO/OTA 15-B) fractures treated with either a Low-Contact Dynamic Compression (LCDC) plate or Pre-contoured Locking (PCL) plate were included. Results. 144 subjects were included in the study. The mean age was 36 years (95% CI 33 38 years) and the mean duration of follow-up was 60 weeks (95% CI 46 74 weeks). 60% of included fractures were wedge pattern (15-B2), followed by 35% simple (15-B1) fractures, and 5% complex (15-B3) fracture patterns. Pre-contoured locking plates were used in 92 cases (64%) and LCDC plates were used in the remaining 52 subjects (36%). 21 subjects (15%) underwent reoperation: 17% of subjects treated with LCDC plates and 13% of subjects treated with PCL plates (p=0.62). Indications for reoperation included painful hardware (86%), hardware failure (9%), and refracture (5%). There was no association between reoperation and age (p = 0.23), gender (p = 0.56), fracture type (p = 0.53), or plate design (p = 0.49). Conclusion. This study represents a large series of displaced clavicle fractures treated with open reduction and plate fixation. Reoperation following plate fixation is relatively common and is primarily due to painful hardware. No difference in reoperation rates between LCDC and pre-contoured plates could be detected in the current sample size

Purpose. Limited information is available regarding the functional outcomes of radial head fractures managed with open reduction and internal fixation (ORIF). The purpose of this study was to determine the functional outcomes of radial head fractures treated with ORIF. Method. Fifty-two patients, with a mean age of 4412 years, who were treated with radial head ORIF were evaluated at a mean of 4.42.4 years. Thirty were isolated radial head fractures (Group A), 13 (Group B) were associated with a complex fracture-dislocation (terrible triad variants), and 5 (Group C) were associated with a proximal ulnar fracture (Monteggia/trans-olecranon variants). Fourty-four were partial articular fractures and 8 were complete articular fractures. Outcomes were assessed with physical and radiographic examination, and validated self-reported questionnaires. Results. The average PREE score (Patient Rated Elbow Evaluation) for Groups A, B, and C were 7.613.1, 12.313.4, and 108.5, respectively. The average MEPI (Mayo Elbow Performance Score) for Groups A, B, and C were 8913, 8511, and 918, respectively. For Groups A, B, and C respectively, the prevalence of radiographic radiocapitellar arthritis was 30%, 46%, and 20%. The average flexion/extension arc for Groups A, B, and C were 7 to 132, 6 to 134, and 10 to 132 respectively. Secondary surgery was performed in 17% of cases, most commonly for decreased motion. Three comminuted fractures failed ORIF and required conversion to radial head arthroplasty. Conclusion. Patients with radial head fractures, including those associated with complex fracture-dislocations, can achieve excellent functional outcomes with low self reported pain and disability when treated with ORIF, despite radiographic evidence of mild post-traumatic arthritis

Background. A consecutive series of 76 patients (101 hips) underwent primary open reduction, capsulorrhaphy and innominate osteotomy for late presenting developmental hip dislocation. They were aged 1.5 to 5 years at the time of surgery between 1958 and 1965. This study was designed to review their outcome into middle age. Methods. We located and reviewed 60 patients (80 hips) using a public records search. This represents a 79% rate of follow-up at 40-48 years post-operatively. 19 patients (24 hips) had undergone total hip replacement and 3 had died. The remaining 38 patients (53 surviving hips) were assessed by the WOMAC. ¯. and Oxford hip outcome questionnaires, physical examination and standing pelvic radiograph. The radiographs were analysed for minimum joint space width and the Kellgren and Lawrence score. Accepted indices of hip dysplasia were measured. Results. Kaplan-Meier survival analysis is presented using the end point of total hip replacement. Survival rates at 30, 40 and 45 years post-reduction are 99% (95% CI +/−2.4), 86% (+/− 6.9) and 54% (+/−16.4) respectively. Average Oxford and WOMAC. ¯. scores for surviving hips were 16.8 (range 0-82) and 16.7 (range 0-71) respectively. Of 51 surviving hip radiographs, 38 had a minimum joint space width in excess of 2.0mm, 13 had definite osteoarthritis (OA) on this criterion. 29 were Kellgren and Lawrence grade 0/1(no or doubtful signs of OA), 7 grade 2 (mild OA), 15 grade 3 or 4 (moderate or severe OA). The average centre-edge and acetabular angles were 40° (range 0-61°) and 32° (20-43°) respectively. There was no significant association between outcome and the modifiable risk factors of body mass or age at surgery. Conclusion. This method of treatment achieves a 54% rate of hip survival at 45 years. Two thirds of surviving hips have an excellent prognosis at this stage

Open reduction and internal fixation of tibial plateau fractures is traditionally performed through an anterior, anterolateral or an anteromedial approach and more recently a posteromedial approach. These approaches allow satisfactory access to the majority of fracture patterns with the exception of posterolateral tibial plateau fractures. To improve access to posterolateral tibial plateau fractures, we developed a posterolateral transfibular neck approach that exposes the tibial plateau between the posterior margin of the iliotibial band and the PCL. The approach can be combined with a posteromedial and/or an anteromedial approach to the tibial plateau. Since April 2007, we have used this approach to treat nine posterolateral tibial plateau fractures. All cases were followed up prospectively. Fracture reduction was assessed on radiographs, CT scans and arthroscopicaly. Maintenance of fracture reduction was assessed with radiostereometric analysis. Clinical outcomes were measured using Lysholm and KOOS scores. Anatomic or near anatomic reduction was achieved in all cases. All fractures healed uneventfully and no loss of osteotomy or tibial plateau fracture reduction was identified on postoperative plain X-rays. In the cases monitored with radiostereometric analysis, the fracture fragments displaced less than 2 mm during the course of healing. All osteotomies healed either at the same rate or quicker than the tibial plateau fractures. There were no signs and no symptoms of lateral or posterolateral instability of the knee during or after the healing of the osteotomy. There were no complications related to the surgical approach, including the fibular head osteotomy. All wounds healed uneventfully and there were no symptoms related to the CPN. The patient reported outcomes recorded for this group at six months, using the Lysholm score (mean 71, median 77, range 42–95), compared favourably to the entire cohort of 33 patients treated operatively at our institution for a tibial plateau fracture and followed up prospectively during the same time period (mean 64, median 74, range 20–100). The posterolateral transfibular approach for lateral tibial plateau fractures is an approach that should be considered for a certain specific pattern of fractures of the lateral tibial plateau. Our preliminary results demonstrated no complications through the learning curve of the development of this technique

We reported a case of the acetabular depression fracture in conjunction with a central fracture dislocation of the hip that was treated with a unique surgical technique. CASE REPORT:. A 76-year-old man suffered a left acetabular fracture with severe left hip joint pain and walking disability. Acetabular fracture was not apparent on the initial radiographs including anteroposterior and oblique views of the pelvis. However, computed tomography (CT) scanning showed displaced acetabular depression fracture (a third fracture fragment) in the center of the weight-bearing area with fracture of the ilium and spontaneous reposition of central dislocation of the hip (Fig. 1, 2). It seemed that this fracture fragment created incongruity of the acetabular articular surface and the potential for hip joint instability. Therefore, the patient was treated with open reduction and internal fixation. SURGICAL TECHNIQUE:. To perform the procedure, the patient was placed in the lateral decubitus position. A direct lateral approach to the hip was used for exposure. The vastus lateralis was released 1 cm distal from its origin, trochanteric osteotomy was done by the Gigli saw. To observe the hip articular surface and to identify the fracture fragment, the femoral head was posterior dislocated with excision of teres ligamentum after T-shaped capsulotomy. The depressed fragment in the acetabulum was identified under direct vision but could not be reduced. Therefore, the outer cortex of the ilium was fenestrated in a size of 2 × 2 cm so that a 1-cm-wide levator was inserted to the depressed fragment at 2 cm proximal from the hip articular surface through the fenestrated window (Fig. 3). Subsequently, the displaced bone fragment was pushed down by using the levator to the adequate articular joint level. The fragment was stabilized with packed cancellous bone graft harvested from the osteotomized greater trochanter. The removed outer cortex of the ilium from fenestrated site was repositioned and fixed by a reconstruction plate and screws. The osteotomized greater trochanter was reattached and fixed with two cannulated cancellous hip screws. RESULTS:. At 9-month follow-up, he was pain-free and continued to function well without the use of external supports. The acetabular depression fracture was completely reduced and healed in the CT scanning evaluation. The patient had no signs of posttraumatic osteoarthritis in radiographs. DISCUSSION and CONCLUSION:. In acetabular fracture dislocations of the hip joint, the precise pathological anatomy is not easily demonstrated by routine radiographs with classification of acetabular fractures. In our case, however, details of acetabular fracture were not well visible on conventional radiographs. It has been shown that computed tomography is useful method in precise evaluation of the fracture type with bone damage and integrity of joint configuration. Concerning approach to the fracture fragment which existed in the center of the weight bearing area of acetabulum, we performed to fenestrate on the intact bony cortex of the ilium just proximal to the fracture site. It was convenient and useful to gain good reduction of the central acetabular depression fracture, although there was no report on such a ‘fenestration’ method

Despite the variety of implants or techniques that exist to treat displaced distal radial fractures, the majority fail to provide sufficient stability to permit early functional recovery. However, locking plates have the advantage over other implants in that locking screws add considerably to the overall stability. The aim of this study was to assess the functional outcome of patients with displaced distal radial fractures treated with a volar distal radial locking plate (Synthes). During a two year period, details of 98 patients admitted to our unit with inherently unstable dorsally displaced distal radial fractures treated with volar locking plates were collected prospectively. For the purpose of this analysis, only those patients (55) with unilateral fracture, able to attend the study clinic at 6 months post-injury were considered. Patients were immobilised in wool and crepe for a 2 week period. The group consisted of 15 males and 40 females with an average age of 54 (28 to 83). At 6 months, patients' perceived functional recovery averaged 80%. Objective assessment was considered in relation to the uninjured side: grip strength 73%; pinch strength 83%; palmarflexion 77%, dorsiflexion 80%; radial deviation 74%; ulnar deviation 74%; pronation 93%, and supination 92%. Seven patients complained of symptoms relating to prominent metalwork. Good/excellent early subjective and objective functional recovery was made following open reduction and internal fixation using volar locking plates of dorsally displaced distal radial fractures. We suggest that objective assessment of grip strength and dorsiflexion can be used as a measure of patient perception of function

BACKGROUND

In our institution we use the Winklestabile volar locking plate for operative fixation of distal radius fractures. This study aims to assess subjective and objective outcomes using this method of treatment.

METHODS

A total of 21 patients who underwent ORIF of distal radius fractures with the Winklestable plate in 2005 with a minimum follow up of 12 months were assessed using the Patient Rated Wrist Evaluation (PRWE) questionnaire and the Disabilities of the Arm Shoulder and Hand (DASH) score. Range of wrist movement (ROM), grip strength and pinchgrip strength were assessed by comparison with the unaffected wrist.

To identify the demographics, mechanisms of injury and treatment modalities of patients presenting at CHBAH with calcaneal fractures. A retrospective study of all admitted patients with calcaneal fractures from 01 January 2018 to 31 December 2022. The data collected includes age, gender, side of injury, mechanism of injury, isolated injury versus polytrauma, open and close fractures, different treatment modalities and time to surgery. The descriptive data will be analysed, with results presented as frequency charts, histograms, and percentages. A total of 98 patients showed a male predominance of 90.8% compared to 9.2% of females. The median age was 38 years, the commonest mechanism was from falls from height, accounting for 67.3% with gunshot injuries accounting for 5%. Bilateral fractures were observed in 5 patients, with similar total numbers between those isolated to the left (47) as compared to the injuries isolated to the right side (46). Calcaneus fractures associated with other injuries in polytrauma cases were observed in 17.4%, and open fractures were observed in 7 patients with 5 having occurred post gunshot injury. A total of 48 patient required surgical treatment with the average time to surgery found to be 15 days. The surgical management varied, with open reduction and internal fixation with plate and screws in 68.7%, followed by 16.7% of patients who were treated with open reduction and internal screw fixation. Calcaneal fractures occurred frequently amongst male patients secondary to fall from height. Half of the total patient number admitted had require a surgical intervention to treat the fracture, with open reduction and internal fixation with plate and screws being the frequent mode of fixation in this population group

Introduction. The condition of the soft tissues surrounding an ankle fracture influences timing and treatment of injuries. Conventional treatment used an open approach to facilitate anatomical reduction and rigid internal fixation. Intramedullary devices for fibular fractures provide a safe alternative in patients in which the condition of the soft tissue envelope or the patient's co-morbidities may benefit from a less invasive approach. We compared outcomes for patients treated with open reduction internal fixation (ORIF) with those undergoing treatment with fibular nails (FN). Methods. 13 consecutive patients treated with fibular nails (FN) were compared with 13 age-matched patients that underwent open reduction and internal fixation (ORIF). All patients were followed to union. Study outcomes were time from admission to surgery, length of stay, wound failure, implant failure, revision surgery, OMAS and SF-36. Results. There was no difference in age or sex distribution between groups. There was no difference in OMAS at 1 year (83 ± 9 in FN group; 80± 21 in ORIF group) and SF-36 (94 ± 11 and 90 ± 20). There were 2 implant failures in the ORIF group and none in the FN group. There was one wound failure in the ORIF group and none in the FN group. Patients treated with FN had a shorter time to surgery (1 day ± 24 hours vs 3 days ± 24 hours) and shorter length of stay (1 day ± 24 hours vs 4 days ± 96 hours). Conclusion. FN is a safe method to treat patients with displaced distal fibular fractures that may have a poor soft tissue envelope and risk factors for wound healing. FN reduces the time to surgery and overall length of stay compared with similar patients treated with conventional ORIF

Olecranon fractures are common injuries representing roughly 5% of pediatric elbow fractures. The traditional surgical management is open reduction and internal fixation with a tension band technique where the pins are buried under the skin and tamped into the triceps. We have used a modification of this technique, where the pins have been left out of the skin to be removed in clinic. The purpose of the current study is to compare the outcomes of surgically treated olecranon fractures using a tension-band technique with buried k-wires (PINS IN) versus percutaneous k-wires (PINS OUT). We performed a retrospective chart review on all pediatric patients (18 years of age or less) with olecranon fractures that were surgically treated at a pediatric academic center between 2015 to present. Fractures were identified using ICD-10 codes and manually identified for those with an isolated olecranon fracture. Patients were excluded if they had polytrauma, metabolic bone disease, were treated non-op or if a non-tension band technique was used (ex: plate/screws). Patients were then divided into 2 groups, olecranon fractures using a tension-band technique with buried k-wires (PINS IN) and with percutaneous k-wires (PINS OUT). In the PINS OUT group, the k-wires were removed in clinic at the surgeon's discretion once adequate fracture healing was identified. The 2 groups were then compared for demographics, time to mobilization, fracture healing, complications and return to OR. A total of 35 patients met inclusion criteria. There were 28 patients in the PINS IN group with an average age of 12.8 years, of which 82% male and 43% fractured their right olecranon. There were 7 patients in the PINS OUT group with an average age of 12.6 years, of which 57% were male and 43% fractured their right olecranon. All patients in both groups were treated with open reduction internal fixation with a tension band-technique. In the PINS IN group, 64% were treated with 2.0 k-wires and various materials for the tension band (82% suture, 18% cerclage wire). In the PINS OUT group, 71% were treated with 2.0 k-wires and all were treated with sutures for the tension band. The PINS IN group were faster to mobilize (3.4 weeks (range 2-5 weeks) vs 5 weeks (range 4-7 weeks) p=0.01) but had a significantly higher complications rate compared to the PINS OUT group (6 vs 0, p =0.0001) and a significantly higher return to OR (71% vs 0%, p=0.0001), mainly for hardware irritation or limited range of motion. All fractures healed in both groups within 7 weeks. Pediatric olecranon fractures treated with a suture tension-band technique and k-wires left percutaneously is a safe and alternative technique compared to the traditional buried k-wires technique. The PINS OUT technique, although needing longer immobilization, could lead to less complications and decreased return to the OR due to irritation and limited ROM
