along the anatomical axis, potentially creating lateral shift of
the mechanical axis. We aimed to determine whether femoral lengthening
along the anatomical axis has an inadvertent effect on lower limb
alignment. Isolated femoral lengthening using the Intramedullary
Skeletal Kinetic Distractor was performed in 27 femora in 24 patients
(mean age 32 years (16 to 57)). Patients who underwent simultaneous realignment
procedures or concurrent tibial lengthening, or who developed mal-
or nonunion, were excluded. Pre-operative and six-month post-operative
radiographs were used to measure lower limb alignment. The mean lengthening
achieved was 4.4 cm (1.5 to 8.0). In 26 of 27 limbs, the mechanical
axis shifted laterally by a mean of 1.0 mm/cm of lengthening (0
to 3.5). In one femur that was initially in varus, a 3 mm medial
shift occurred during a lengthening of 2.2 cm. In a normally aligned limb, intramedullary lengthening along
the anatomical axis of the femur results in a lateral shift of the
mechanical axis by approximately 1 mm for each 1 cm of lengthening.
Aims. Temporary hemiepiphysiodesis (HED) is applied to children and adolescents to correct angular deformities (ADs) in long bones through guided growth. Traditional Blount staples or two-hole plates are mainly used for this indication. Despite precise surgical techniques and attentive postoperative follow-up, implant-associated complications are frequently described. To address these pitfalls, a flexible staple was developed to combine the advantages of the established implants. This study provides the first results of guided growth using the new implant and compares these with the established two-hole plates and Blount staples. Methods. Between January 2013 and December 2016, 138 patients (22 children, 116 adolescents) with genu valgum or genu varum were treated with 285 flexible staples. The minimum follow-up was 24 months. Introduction. Intramedullary lengthening devices have been in use in older children with closed /open growth plates with good success. This study aims to present the early experience of the FITBONE nail since withdrawal of the PRECICE nail. Materials & Methods. Retrospective analysis of both antegrade and retrograde techniques were utilized. Only patients where union was achieved and full weight bearing commenced were included. The complication rate, length gained, distraction index, weight bearing index (WBI) as well as mechanical axes were analysed. Results. 14 (7 males, 7 females) of a total of 16 (7 males, 7 females) patients with a mean age of 16.9 years with varied diagnosis of LLD were analysed. The mean length gained was 38 mm with an average distraction index of 0,74 mm/day. WBI in these patients on average was 59,6 days/cm lengthened. 6 complications were observed, including two nonunions (successfully treated) and a knee subluxation.
Unicompartmental knee arthroplasty (UKA) and high tibial osteotomy (HTO) are well-established operative interventions in the treatment of knee osteoarthritis (KOA). However, which of these interventions is more beneficial, to patients with KOA, is not known and remains a topic of much debate. Aims: (i) To determine whether UKA or HTO is more beneficial in the treatment of isolated medial compartment KOA, via an assessment of patient-reported outcome measures (PROMs). (ii) To investigate the relationship between PROMs and radiographic parameters of knee joint orientation/alignment. This longitudinal observational study assessed a total of 42 patients that had undergone UKA (n=23) or HTO (n=19) to treat isolated medial compartment KOA. The PROMs assessed, pre-operatively and 1-year post-operatively, consisted of the: self-administered comorbidity questionnaire; short form-12; oxford knee score; knee injury and osteoarthritis outcome score; and the EQ-5D-5L. Introduction. Computer hexapod assisted orthopaedic surgery (CHAOS) has previously been shown to provide a predictable and safe method for correcting multiplanar femoral deformity. We report the outcomes of tibial deformity correction using CHAOS, as well as a new cohort of femoral CHAOS procedures. Materials and Methods. Retrospective review of medical records and radiographs for patients who underwent CHAOS for lower limb deformity at our tertiary centre between 2012–2020. Results. There were 70 consecutive cases from 56 patients with no loss to follow-up. Mean age was 40 years (17 to 77); 59% male. There were 48 femoral and 22 tibial procedures. Introduction. To evaluate the results of correction of knee deformities based on deformity analysis in Achondroplasia, the commonest skeletal dysplasia as some have concomitant ligamentous deformities. Materials and Methods. Retrospective study from a prospective database (2007–2020) of achondroplasts who underwent growth modulation. Analysis of medical records with objective measurement of mechanical axis radiographs was done (Traumacad). Satisfactory alignment was defined as neutral to slightly varus (0–15 mm MAD) so that the MCL/LCL laxity is not revealed. Results. 23 patients, 41 limbs, 34 bilateral, 6 unilateral underwent multiple growth modulation procedures. 2 had valgus knees. 15 patients underwent proximal fibular epiphysiodesis in addition for LCL laxity with one isolated fibular epiphysiodesis.

Background. Achieving optimal prosthesis alignment during total knee arthroplasty (TKA) is essential. Imageless computer-assisted surgery (CAS) is developed to improve knee prosthesis alignment and with CAS it is possible to perform intraoperative alignment measurements. Lower limb alignment measurements are also performed for preoperative planning and postoperative evaluation. A new stereoradiography system, called EOS, can be used to perform these measurements in 3D and thus measurement errors due to malpositioning can be eliminated. Since both CAS and EOS are based on 3D modeling, measurements should theoretically correlate well. Therefore, objective was to compare intraoperative CAS-TKA measurements with pre- and postoperative EOS 3D measurements. Methods. In a prospective study 56 CAS-TKAs were performed and alignment measurements were recorded two times: before bone cuts were made and after implantation of the prosthesis. Pre- and postoperative coronal alignment measurements were performed using EOS 3D. CAS measurements were compared with EOS 3D reconstructions. Measured angles were: varus/valgus (VV), mechanical lateral distal-femoral (mLDFA) and medial proximal tibial angle (mMPTA). Results. Significantly different VV angles were measured pre- and postoperatively with CAS compared to EOS. For preoperative measurements, mLDFA did not differ significantly, but a significantly larger mMPTA in valgus was measured with CAS. Conclusions. Despite multiple published reviews, the optimum method of correction and stabilisation of Blount's disease remains controversial. The purpose of this study is to evaluate the clinical and radiological outcomes of acute correction of late-onset tibial vara by percutaneous proximal tibial osteotomy with circular external fixation using two simple rings. This technique was developed to minimise cost in a context of limited resources. This study was conducted between 2016 and 2020. We retrospectively reviewed the clinical notes and radiographs of 30 patients (32tibiae) who had correction of late-onset tibia by proximal tibial osteotomy and Ilizarov external fixator. C-Arm fluoroscopy is limited by its 2D imaging modality and is incapable of providing accurate 3D quantitative assessment of operative anatomy. In High Tibial Osteotomy (HTO), assessing the distance between the mechanical axis of the leg and the centre of the knee joint is difficult to accomplish due to limited fluoroscopic view size. A previously developed sensor-based tracking system (TC-Arm)adds on to C-arm equipment to provide additional quantitative capabilities. A new image-based tracking module was developed for TC-Arm using a reference panel with an array of fiducial markers. The image analysis software segments the marker positions in each image and identifies image coordinates with respect to the panel. Each image's parameters are identified by 2D-3D matching of the panel's 3D model to the marker's epipolar geometries. Finally, the defined linear transformation matrices are applied for positioning all the fluoroscopic images with respect to the same global reference. A Sawbone model of the leg was used as a phantom and marked with radio-dense fiducial markers at the centres of each joint. Introduction and Objective. Malunion after trauma can lead to coronal plane malalignment in the lower limb. The mechanical hypothesis suggests that this alters the load distribution in the knee joint and that that this increased load may predispose to compartmental arthritis. This is generally accepted in the orthopaedic community and serves as the basis guiding deformity correction after malunion as well as congenital or insidious onset malalignment. Much of the literature surrounding the contribution of lower limb alignment to arthritis comes from cohort studies of incident osteoarthritis. There has been a causation dilemma perpetuated in a number of studies - suggesting malalignment does not contribute to, but is instead a consequence of, compartmental arthritis. In this investigation the relationship between compartmental (medial or lateral) arthritis and coronal plane malalignment (varus or valgus) in patients with post traumatic unilateral limb deformity was examined. This represents a specific niche cohort of patients in which worsened compartmental knee arthritis after extra-articular injury must rationally be attributed to malalignment. Materials and Methods. Recent literature as pointed out the patients with «constitutional varus »; in these cases, restoring neutral alignment could be abnormal and even undesirable. We present the results of the surgical correction of lower-limb deformities caused by metabolic bone disease. Our series consisted of 17 patients with a diagnosis of hypophosphataemic rickets and two with renal osteodystrophy; their mean age was 25.6 years (14 to 57). In all, 43 lower-limb segments (27 femora and 16 tibiae) were osteotomised and the deformity corrected using a monolateral external fixator. The aim of this study was to compare the accuracy of limb alignment and component positioning after total knee arthroplasty(TKA) performed using fixed or individual distal femoral valgus correction angle(VCA)in valgus knees. Materials and Methods. One hundred and twenty-four patients were randomised to undergo TKA with either of the clinical baseline, radiological outcomes and subsequent outcome such as knee HSS scores, knee range of motion (ROM) and visual analogue scale (VAS) scores were assessed.
