Total hip arthroplasty (THA) is an effective treatment for symptomatic hip osteoarthritis (OA). While computer-navigation technologies in total knee arthroplasty show survivorship advantages and are widely used, comparable applications within THA show far lower utilisation. Using national registry data, this study compared patient reported outcome measures (PROMs) in patients who underwent THA with and without computer navigation. Data from Australian Orthopaedic Association National Joint Replacement Registry (AOANJRR) PROMs program included all primary THA procedures performed for OA up to 31 December 2020. Procedures using the Intellijoint HIP® navigation system were identified and compared to procedures using other computer navigation systems or conventional instrumentation only. Changes in PROM scores between pre-operative and 6-month post-operative time points were analysed using multiple regression model, adjusting for pre-operative score, patient age, gender, ASA score, BMI, surgical approach, and hospital type. There were 65 primary THA procedures that used the Intellijoint HIP® system, 90 procedures used other types of computer navigation, and the remaining 5,284 primary THA procedures used conventional instrumentation. The estimated mean changes in the EuroQol visual analogue scale (EQ VAS) score and Oxford Hip score did not differ significantly when Intellijoint® was compared to conventional instruments (estimated differences of 2.4, 95% CI [-1.7, 6.5], p = 0.245, and −0.5, 95% CI [-2.5, 1.4], p = 0.592, respectively). The proportion of patients who were satisfied with their procedure was also similar when Intellijoint® was compared to conventional instruments (rate ratio 1.06, 95% CI [0.97, 1.16], p = 0.227). The preliminary data demonstrate no significant difference in PROMs when comparing the Intellijoint HIP® THA navigation system with both other navigation systems and conventional instrumentation for primary THAs performed for OA. Total hip arthroplasty (THA) is an effective treatment for symptomatic hip osteoarthritis (OA). Computer-navigation technologies in total knee arthroplasty show evidence-supported survivorship advantages and are used widely. The aim of this study was to determine the revision outcome of hip commercially available navigation technologies. Data from the Australian Orthopaedic Association National Joint Replacement Registry from January 2016 to December 2020 included all primary THA procedures performed for osteoarthritis (OA). Procedures using the Intellijoint HIP® navigation were identified and compared to procedures inserted using ‘other’ computer navigation systems and to all non-navigated procedures. The cumulative percent revision (CPR) was compared between the three groups using Kaplan-Meier estimates of survivorship and hazard ratios (HR) from Cox proportional hazards models, adjusted for age and gender. A prosthesis specific analysis was also performed. There were 1911 procedures that used the Intellijoint® system, 4081 used ‘other’ computer navigation, and 160,661 were non-navigated. The all-cause 2-year CPR rate for the Intellijoint HIP® system was 1.8% (95% CI 1.2, 2.6), compared to 2.2% (95% CI 1.8, 2.8) for other navigated and 2.2% (95% CI 2.1, 2.3) for non-navigated cases. A prosthesis specific analysis identified the Paragon/Acetabular Shell THAs combined with the Intellijoint HIP® system as having a higher (3.4%) rate of revision than non-navigated THAs (HR = 2.00 (1.01, 4.00), p=0.048). When this outlier combination was excluded, the Intellijoint® system group demonstrated a two-year CPR of 1.3%. There was no statistical difference in the CPR between the three groups before or after excluding Paragon/Acetabular Shell system. The preliminary data presented demonstrate no statistical difference in all cause revision rates when comparing the Intellijoint HIP® THA navigation system with ‘other’ navigation systems and ‘non-navigated’ approaches for primary THAs performed for OA. The current sample size remains too small to permit meaningful subgroup statistical comparisons

Introduction. Interactions between hip, pelvis and spine, as abnormal spinopelvic movements, have been associated with inferior outcomes following total hip arthroplasty (THA). Changes in pelvis position lead to a mutual change in functional cup orientation, with both pelvic tilt and rotation having a significant effect on version. Hip osteoarthritis (OA) patients have shown reduced hip kinematics which may place increased demands on the pelvis and the spine. Sagittal and coronal planes assessments are commonly done as these can be adequately studied with anteroposterior and lateral radiographs. However, abnormal pelvis rotation is likely to compromise the outcome as they have a detrimental effect on cup orientation and increased impingement risk. This study aims to determine the association between dynamic motion and radiographic sagittal assessments; and examine the association between axial and sagittal spinal and pelvic kinematics between hip OA patients and healthy controls (CTRL). Methods. This is a prospective study, IRB approved. Twenty hip OA pre-THA patients (11F/9M, 67±9 years) and six CTRL (3F/3M, 46±18 years) underwent lateral spinopelvic radiographs in standing and seated bend-and-reach (SBR) positions. Pelvic tilt (PT), pelvic-femoral-angle (PFA) and lumbar lordosis (LL) angles were measured in both positions and the differences (Δ) between standing and SBR were calculated. Dynamic SBR and seated maximal-trunk-rotation (STR) were recorded in the biomechanics laboratory using a 10-infrared camera and processed on a motion capture system (Vicon, UK). Direct kinematics extracted maximal pelvic tilt (PT. max. ), hip flexion (HF. max. ) and (mid-thoracic to lumbar) spinal flexion (SF. max. ). The SBR pelvic movement contribution (ΔPT. rel. ) was calculated as ΔPT/(ΔPT+ΔPFA)∗100 for the radiographic analysis and as PT. max. /(PT. max. +HF. max. ) for the motion analyses. Axial and sagittal, pelvic and spinal range of motion (ROM) were calculated for STR and SBR, respectively. Spearman's rank-order determined correlations between the spinopelvic radiographs and sagittal kinematics, and the sagittal/axial kinematics. Mann-Whitney U-tests compared measures between groups. Results. Radiograph readings correlated with sagittal kinematics during SBR for ΔPT and PT. max. (ρ=0.64, p<0.001), ΔPFA and HF. max. (ρ=0.44, p<0.0002), and ΔLL and SF. max. (ρ=0.34, p=0.002). Relative pelvic movements (ΔPT. rel. ) were not different between radiographic (11%±21) and biomechanical (15%±29) readings (p=0.9). Sagittal SRB spinal flexion correlated with the axial STR rotation (ρ=0.43, p<0.0001). Although not seen in CTRL, sagittal SRB pelvic flexion strongly correlated with STR pelvic rotation in OA patients (ρ=0.40, p=0.002). All spinopelvic parameters were different between the patients with OA and CTRL. CTRLs exhibited significantly greater mobility and less variability in all 3 segments (spine, pelvis, hip) and both planes (axial and sagittal) (Table 1). Conclusion. Correlation between sagittal kinematics and radiographical measurements during SBR validates the spinopelvic mobility assessments in the biomechanics laboratory. Axial kinematics of both pelvis and spine correlated significantly in OA patients, suggesting that patients with abnormal sagittal mobility are likely to also exhibit abnormal axial mobility, which can further potentiate any at-risk kinematics. Significantly lower OA ROM must be investigated post-THA. Pre-THA variability of both sagittal and axial movements indicates that both planes must be considered ahead of surgical planning with navigation and/or robotics. Total hip arthroplasty (THA) is one of the most successful and effective treatments for advanced hip osteoarthritis (OA). Over the last 5 years, Canada has seen a 17.8% increase in the number of hip replacements performed annually, and that number is expected to grow along with the aging Canadian population. However, the rise in THA surgery is associated with an increased number of patients at risk for the development of an infection involving the joint prosthesis and adjacent deep tissue – periprosthetic joint infections (PJI). Despite improved hygiene protocols and novel surgical strategies, PJI remains a serious complication. No previous population-based studies has investigated PJI risk factors using a time-to-event approach and none have focused exclusively on patients undergoing THA for primary hip OA. The purpose of this study is to determine risk factors for PJI after primary THA for OA using a large population-based database collected over 15 years. Our secondary objective is to determine the incidence of PJI, the time to PJI following primary THA, and if PJI rates have changed in the past 15 years. We performed a population-based cohort study using linked administrative databases in Ontario, Canada in accordance with RECORD and STROBE guidelines. All primary total hip replacements performed for osteoarthritis in patients aged 55 or older between January 1st 2002 – December 31st 2016 in Ontario, Canada were identified. Periprosthetic joint infection as the cause for revision surgery was identified with the International Classification of Diseases, 10th Edition (ICD-10), Clinical Modification diagnosis code T84.53 in any component of the healthcare data set. Data were obtained from the Institute for Clinical Evaluative Sciences (ICES). Demographic data and outcomes are summarized using descriptive statistics. We used a Cox proportional hazards model to analyze the effect of surgical factors and patient factors on the risk of developing PJI. Surgical factors include the approach, use of bone graft, use of cement, and the year of surgery. Patient factors include sex, age at surgery, income quintile and rurality (community vs. urban). We compared the 1,2,5 and 10 year PJI rates for patients undergoing THA each year of our cohort with the Cochran-Armitage test. Less than 0.1% of data were missing from all fields except for rurality which was lacking 0.3% of data. A total of 100,674 patients aged 55 or older received a primary total hip arthroplasty for osteoarthritis from 2002–2016. We identified 1034 cases of revision surgery for prosthetic joint infection for an overall PJI rate of 1.03%. When accounting for patients censored at final follow-up, the cumulative incidence for PJI is 1.44%. Our Cox proportional hazards model revealed that male sex, Type II diabetes mellitus, discharge to convalescent care, and having both hips replaced during one's lifetime were associated with increased risk of developing PJI following primary THA. Importantly, the time adjusted risk for PJI was equal for patients operated within the past 5 years, 6–10 years ago, or 11–15 years ago. The surgical approach, use of bone grafting or cement were not associated with increased risk of infection. PJI rates have not changed significantly over the past 15 years. One, two, five and ten-year PJI rates were similar for patients undergoing THA in all qualifying years. Analysis of a population-based cohort of 100,674 patients has shown that the risk of developing PJI following primary THA has not changed over 15 years. The surgical approach, use of bone grafting or cement were not associated with increased risk of infection. Male sex, Type II diabetes Mellitus and discharge to a rehab facility are associated with increased risk of PJI. As the risk of PJI has not changed in 15 years, an appropriately powered trial is warranted to determine interventions that can improve infection rate after THA

Introduction. Hip osteoarthritis (OA) results in reduced hip range of motion and contracture, affecting sitting and standing posture. Spinal pathology such as fusion or deformity may alter the ability to compensate for reduced joint mobility in sitting and standing postures. The effects of postural spinal alignment change between sitting and standing is not well understood. Methods. A retrospective radiographic review was performed at a single academic institution of patients with sitting and standing full-body radiographs between 2012 and 2017. Patients were excluded if they had transitional lumbosacral anatomy, prior spinal fusion or hip prosthesis. Hip OA severity was graded by the Kellgren-Lawrence grades and divided into two groups: low-grade OA (LOA; grade 0–2) and severe OA (SOA; grade 3–4). Spinopelvic parameters (Pelvic Incidence (PI), Pelvic Tilt (PT), Lumbar Lordosis (LL), and PI-LL), Thoracic Kyphosis (TK; T4-T12), Global spinal alignment (SVA and T1-Pelvic Angle; TPA; T10-L2) as well as proximal femoral shaft angle (PFSA: as measured from the vertical), and hip flexion (difference between change in PT and change in PFSA) were also measured. Changes in sit-stand radiographic parameters were compared between the LOA and SOA groups with unpaired t-test. Results. 548 patients were identified with sit-stand radiographs, of which there were 311 patients with LOA & 237 with SOA. After propensity score matching for Age, BMI, and PI, 183 LOA & 183 SOA patients were analyzed. Standing alignment analysis demonstrated that SOA patients had greater SVA (31.1 ± 36.68 vs 21.7 ± 38.83, p=0.02), and lower TK (−36.21 ± 11.98 vs −41.09 ± 11.47, p<0.001). SOA patients had lower PT, greater PI-LL, lower LL, lower T10-L2, and lower TPA (p>0.05). PFSA (9.09 5.19 vs 7.41 4.48, p<0.001) was significantly different compared to LOA while SOA KA was not significantly different compared to LOA. Sitting alignment analysis demonstrated that SOA patients had higher PT (29.69 ± 15.65 vs 23.32 ± 12.12, p<0.001), higher PI-LL (21.64 ±17.86 vs 12.44 ±14.84 p<0.001), lower LL (31.67 ± 16.40 vs 41.58 ± 14.73, p<0.001), lower TK (−33.22 ± 15.76 vs −38.57 ± 13.01, p=0.01), greater TPA (27.91 ± 14.7 vs 22.55 ± 11.38 p=0.01). TK, SVA, and PFSA were not significantly different compared to LOA. SOA and LOA groups demonstrated differences in standing and sitting spinopelvic alignment for all global and regional parameters except PI. When examining the postural changes from standing to sitting, there was less hip ROM in SOA than LOA (71.45 ± 18.55 vs 81.64 ± 12.57, p<0.001). As a result, SOA patients had more change in PT (15.24 ± 16.32 vs 7.28 ± 10.19, p<0.001), PI-LL (20.62 ± 17.25 vs 13.74 ± 11.16, p<0.001), LL (−21.37 ± 15.55 vs −13.09 ± 12.34, p<0.001), and T10-L2 (−4.94 ± 7.45 vs −1.08 ± 5.19, p<0.001) to compensate. SOA had a greater improvement in TPA (15.06 vs 9.59, p<0.001), and less change in PFSA (86.65 vs 88.81, p<0.001) compared to LOA. Conclusions. Spinopelvic compensatory mechanisms are adapted for reduced joint mobility associated with hip OA in standing and sitting

Background. Total Hip Arthroplasty (THA) has long been the standard treatment for cases in which non-surgical alternatives have failed to improve pain and function in hip osteoarthritis (OA) patients. Outcomes from THA have improved over time with better surgical techniques and improved implant designs. While conventional neck-sacrificing implants have been associated with favorable outcomes, there is evidence to suggest biomechanical advantages of newer, femoral neck-preserving short-stem implants, including the Corin MiniHip. However, there is a still a gap of knowledge regarding the potential benefits of the MiniHip stem over conventional neck-sacrificing stems in regards to patient-reported outcomes (PROs). In this study, we investigated the differences between a neck-sacrificing stem design and neck-preserving short-stem design (MiniHip, Corin Inc.) arthroplasty concerning PROs, and considering the known features of the short stem design, we hypothesized that MiniHip THA would be associated with improved PROs in comparison to a neck-sacrificing implant system. We further sought to investigate gender effects related to MiniHip or conventional stem surgery. Methods. Neck-sacrificing implant patients (n=90, age 57±7.9 years, female=58, male=32) and a matched (matching criteria: follow-up period, BMI, age) cohort group of MiniHip patients (n=105, age 55.16±9.88 years, female: 25, male: 80) reported both pre-operative and post-operative Hip disability and Osteoarthritis Outcome Scores (HOOS) at a minimum interval of 6 months post-operatively and up to three years postoperatively. We applied MANCOVA analysis to compare patient-reported outcome subscores from each group using follow-up period as a covariate and employing gender as an additional grouping factor to evaluate gender effects. Statistical significance was set at α=0.05 and Bonferroni corrections were applied to account for multiple comparisons. Results. There was a main effect of time, showing that all HOOS subscores of both groups increased significantly after surgery (p<0.001). There was a main effect of surgery for subscores Symptoms (p=0.038), ADL (p=0.046), and Sports and Recreation (p=0.039). There was a gender effect only for the subscore Symptoms (p=0.007). There were significant time by surgery interactions for HOOS subscores Symptoms (p=0.002), Pain (p=.009), Sports and Recreation (p=0.004), and QOL (p<0.001) subscores. We also observed a significant time by gender interaction effect for all HOOS subscores (p<0.001). Discussion. The interaction effects regarding most HOOS subscores and surgery/implant type indicate an advantage of MiniHip surgery regarding post-operative reported outcomes. The observed results may be due to previously described improved physiological loading and native hip structure preservation with neck-preserving short-stem designs. While longer-term studies are required for further investigation, evidence suggests the MiniHip may provide a significant benefit to primary THA patients. The additional gender/time interaction effect observed in our study highlights the necessity to consider potential sex differences regarding both the potential/expected improvement in PROs from THA and the requirement to account for such differences when designing osteoarthritis outcome studies based on PROs

Bone remodeling effects is a significant issue in predicting long term stability of hip arthroplasty. It has been frequently observed around the femoral components especially with the implantation of prosthesis stem. Presence of the stiffer materials into the femur has altering the stress distribution and induces changes in the architecture of the bone. Phenomenon of bone resorption and bone thickening are the common reaction in total hip arthroplasty (THA) which leading to stem loosening and instability. The objectives of this study are (i) to develop inhomogeneous model of lower limbs with hip osteoarthritis and THA and (ii) to predict the bone resorption behavior of lower limbs for both cases. Biomechanical evaluations of lower limbs are established using the finite element method in predicting bone remodeling process. Lower limbs CT-based data of 79 years old female with hip osteoarthritis (OA) are used in constructing three dimensional inhomogenous models. The FE model of lower limbs was consisted of sacrum, left and right ilium and both femur shaft. Bond between cartilage, acetabulum and femoral head, sacrum and ilium were assumed to be rigidly connected. The inhomogeneous material properties of the bone are determined from the Hounsfield unit of the CT image using commercial biomedical software. A load case of 60kg body weight was considered and fixed at the distal cut of femoral shaft. For THA lower limbs model, the left femur which suffering for hip OA was cut off and implanted with prosthesis stem. THA implant is designed to be Titanium alloy and Alumina for stem and femoral ball, respectively. Distribution of young modulus of cross-sectional inhomogeneous model is presented in Fig. 2 while model of THA lower limbs also shown in Fig. 2. Higher values of young modulus at the outer part indicate hard or cortical bone. Prediction of bone resorption is discussed with the respect of bone mineral density (BMD). Changes in BMD at initial age to 5 years projection were simulated for hip OA and THA lower limbs models. The results show different pattern of stress distribution and bone mineral density between hip OA lower limbs and THA lower limbs. Stress is defined to be dominant at prosthesis stem while femur experienced less stress and leading to bone resorption. Projection for 5 years follow up shows that the density around the greater tronchanter appears to decrease significantly

Introduction. Subtle deformities of the acetabulum and proximal femur are recognised as biomechanical risk factors for the development of hip osteoarthritis (OA) as well as a cause of hip and groin pain. We undertook this study to examine relationships between a number of morphological measurements of the acetabulum and proximal femur and the hip pain in a 20-year longitudinal study. Methods. In 1989 women of 45–64 years of age were recruited. Each had an AP-Pelvis radiograph at Year-2. These radiographs were analysed using a validated programme for measuring morphology. All morphological measurements were read blinded to outcome. At year 3 all participants were asked whether they experienced hip pain (side specific). This was repeated at visits up to and including 20-years. Logistic regression analysis (with robust standard errors and clustering by subject identifier) was performed using hip pain as a binary outcome. The model adjusted for baseline age, BMI and joint space and included only participants who were pain free on initial questioning. Results. 743 participants were included in the analysis. Median age 74.0. Pain was reported in 14.2% of hips. Logistic regression analyses revealed that extrusion index and LCE were significantly associated with hip pain before and after adjusting for covariates (OR 4.88[95%CI 1.32–17.97, p=0.017] and 0.84[95%CI 0.74–0.96, p=0.012] respectively). Modified triangular index height (MTIH) was also significantly associated after adjusting for covariates (OR 1.10[95%CI 1.01–1.20, p=0.022]). Extrusion index and MTIH were independently associated with hip pain at 20-years when used in the same model. No significant interaction was identified. Conclusions. This study provides evidence that measurements of hip morphology characteristic of previously undiagnosed dysplasia and FAI are predictive of hip pain in a 20-year longitudinal study. MTIH, LCE and Extrusion index were significant predictors of pain. This is the first study to describe these associations between hip morphology and pain in a longitudinal cohort

Introduction. The advance of surgical technique and implant design have led to improvement in total hip arthroplasty (THA), and short stem THA is now gaining number as a treatment option for younger patients to preserve bone in the proximal femur for a future revision. The SMF stem is shorter stem, and requires a slightly higher neck resection and implanted in slight varus to contact at lateral cortex. Developmental dysplasia of the hip (DDH) is the most common cause to hip osteoarthritis (OA) in Japan, and the morphology of the dysplastic femur is narrow canal and increased anteversion. Thus, the purpose of this study is to evaluate the SMF stem design can fit for Japanese patients, using CT based 3-D template planning. Methods. We evaluated 30 patients who required THA in our institution. Inclusion criteria are hip OA, but cases with post-trauma, post-osteotomy, and any other hip disease in childhood are excluded. Patients were selected with their femoral anteversion, based on the dispersion of anteversion in dysplastic hip population, which was reported by Noble and collegues in 2003. Preoperative planning with ZedHip software (Lexi, Japan) was performed by established protocol. The center of socket was placed at 15mm proximal from teardrop and medialized to primary acetabulum, with 40° of radiographic inclination and 20° of anteversion. Neck resection of femur was 20mm proximal from the top of smaller trochanter, and stem was placed with lateral fit at distal and medial fit at calcar with appropriate size. Stem offset was selected by leg extension and balanced shenton line. Finally, ROM simulation was performed and the socket anteversion was arranged to achieve the optimal ROM. And then, SMF stem alignment and appropriate fitting was evaluated in Japanese OA cases. Results. Within 30 cases, 28 were female and 2 were male. Mean age was 66.0±9.6, and 8 (26.7%) were primary OA and 22 (73.3%) were secondary OA, from dysplastic hip. SMF stem was placed at average 2.7° varus and 7.2° flexion against femoral axis [Fig. 1]. The average degree of femoral anteversion was 34.0°±10.9 at pre-implantation and 22.7°±9.9 after implantation, which means average 11.4° retroverted [Fig. 2]. Stem offset and neck length variation was acceptable in 28/30 cases (93.3%). Finally, the mean flexion was 130°±7.1 and abduction was 56.0°±5.5, and optimal ROM was achieved 19/30 cases (63.3%). We cannot find any correlation between femoral anteversion pre-implantion and ROM post-implantation, however positive correlation was found between flexion angle and offset length after implantation (r2=0.48). Conclusion. The stem design of SMF is unique to preserve bone stock, but impossible to control anteversion after implantation, so it should be evaluated if it could fit to dysplastic hip. In this 3-D simulation study, SMF stem can be appropriately fitted in most of Japanese cases. Neck length and offset is important to achieve optimal ROM. Large diameter head (≥32mm) and high-offset stem would be suggested, and it may spread the indication for most of the cases regardless of the anteversion of femur. Greater trochanteric pain syndrome (GTPS) is common, yet the impact on function and quality of life has not been measured. The aim of this study was to quantify the impact on function and quality of life, comparing the results to people with severe hip osteoarthritis and an asymptomatic control group. Forty two people with GTPS – including 11 not actively seeking treatment and 11 seeking surgical treatment, 20 with severe hip osteoarthritis (OA), and 23 age and sex matched asymptomatic participants (ASC) where recruited from public and private hospitals, and the community. Upon confirming meeting inclusion and exclusion criteria participants were interviewed. Exclusion criteria included lumbar nerve root signs; inflammatory, neoplastic and metabolic disorders. Measured used were the Harris hip score (HHS); the Oswestry disability index (ODI); the Australian quality of life instrument (AQoL); the Functional co-morbidity index (FCI); and fulltime work assessments. No difference was found between the GTPS and the OA group on the HHS, ODI, AQoL or the FCI measures. Both symptomatic groups were significantly more disabled than the ASC group on the HHS and ODI (p<0.001). The GTPS and OA groups had lower AQoL than the ASC group (p<0.001); and higher FCI results than the ASC group (GTPS vs ASC, p=0.005; OA vs ASC, p=0.019). GTPS participants were least likely to be in full time work; full time work participation probability (95% C.I.): GTPS Prob=0.288 (0.160 to 0.463), OA Prob= 0.518 (0.273 to 0.753); ASC group of Prob=0.676 (0.439 to 0.847). People with GTPS have similar levels of pain, disability and quality of life, but are less likely to be in full time employment than people with severe hip OA which puts them at risk of economic hardship. Research on conservative and surgical treatments should measure pain, disability and work participation

Background:. Total hip arthroplasty (THA) has been proven to be successful in achieving adequate pain relief and favorable outcomes in patients suffering from hip osteoarthritis (OA). However, leg length discrepancy (LLD) is still a significant cause of morbidities such as nerve damage, low back pain and abnormal gait. Despite most of the reported values of LLD in the literature being within the acceptable threshold of < 10 mm, some patients still report dissatisfaction, leading to litigation against orthopedic surgeons. However, lower extremity lengthening is sometimes necessary to achieve adequate hip joint stability and prevent dislocations. The purpose of this study was to compare LLD in patients undergoing THA using three different techniques: conventional anterior-approach THA (ATHA), conventional posterior-approach THA (PTHA), and robotic-assisted posterior-approach THA (RTHA) using the MAKO™ robotic hip system. Materials and Methods:. All cases of RTHA, ATHA and PTHA that were performed by the senior surgeon between Sep 2008 and Dec 2012 were reviewed. Patients included in this study had a primary diagnosis of hip osteoarthritis, with available and proper post-operative antero-posterior pelvis radiographs. All radiographs were calibrated and measurements done twice by two blinded observers. Results:. After exclusions, 67 RTHA cases, 29 ATHA cases and 59 PTHA cases were included in this study. There was a strong inter- and intra-observer correlation for all LLD measurements (r > 0.9, p < 0.001 in all). Mean LLD in the RTHA, ATHA and PTHA groups was 2.7 ± 1.8 mm (CI. 95. : 2.3, 3.2), 1.8 ± 1.6 mm (CI. 95. : 1.2, 2.4) and 1.9 ± 1.6 mm (CI. 95. : 1.5, 2.4) respectively (p = 0.01). When LLD > 3 mm was set as an outlier, the percentage of patients in the RTHA, ATHA and PTHA groups was 37.3%, 17.2% and 22% respectively (p value range, 0.06–0.78). When LLD > 5 mm was set as an outlier, the percentage of patients in the RTHA, ATHA and PTHA groups was 10.4%, 6.9% and 8.5% respectively (p value range, 0.72–1.0). None of the patients in all three groups had LLD ≥ 10 mm (Figure 1). Conclusion:. The results of the current study demonstrate the accuracy of the MAKO™ system in obtaining minimal LLD compared to the conventional anterior-approach and conventional posterior-approach THA, with no cases having LLD > 10 mm

Although there are several reports of excellent long-term survival after cemented total hip arthroplasty (THA), cemented acetabular components are prone to become loose when compared with femoral components. On the other hand, the survival of cementless acetabular components has been reported to be equal or better than cemented ones and the use of cementless acetabular components is increasing. However, most of the reports on survival after THA are for patients with primary hip osteoarthritis (OA) and there is no report of 20-year survival of cementless THA for patients with hip dysplasia. It is supposed to be more difficult to fix cementless acetabular components for OA secondary to hip dysplasia than primary OA. The purposes of this study were to review retrospectively the 20-year survival of cemented and cementless THA for hip dysplasia and to compare the effect of fixation methods on the long-term survival for patients with hip dysplasia. We retrospectively reviewed all patients with OA secondary to hip dysplasia treated with a cemented Bioceram hip system between 1981 and 1987, and a cementless cancellous metal Lübeck hip system between 1987 and 1991. We excluded patients aged more than 60 years, males, and Crowe 4 hips. The studied subjects were 70 hips of cemented THA (Group-C) and 57 hips of cementless THA (Group-UC). Both hip implants had a 28-mm alumina head on polyethylene articulation. The mean age at operation was 50.5 years (range, 36–60 years) in Group-C and 50.0 years (range, 29–60 years) in Group-UC. The mean BMI was 23.2 kg/m. 2. in Group-C (range, 17.3–29.3 kg/m. 2. ) and 22.9 kg/m. 2. in Group-UC (range, 18.8–28.0 kg/m. 2. ). There were no significant differences in age and BMI between the two groups. The average follow-up period was 18.0 years in Group-C and 18.4 years in Group-UC. In Group-C, revision was performed in 33 hips due to aseptic cup loosening (30 hips), stem loosening (one hip), and loosening of both components (two hips). In Group-UC, revision was performed in 10 hips due to stem fracture secondary to distal fixation (4 hips), cup loosening (three hips), polyethylene breakage (two hips), and extensive osteolysis around the stem (one hip). The survival at 20 years regarding any revision as the endpoint was 51% in Group-C and 84% in Group-UC. This difference was significant using Log-rank test (P=0.006). The cup survival at 20 years was 54% in Group-C and 92% in Group-UC. This difference was also significant (P = 0.0003). The stem survival at 20 years was 95% in Group-C and 92% in Group-UC. This difference was not significant (P = 0.4826). Cementless THA showed a higher survival rate at 20 years for hip dysplasia than cemented THA because of the excellent survival of the acetabular component without cement. We conclude that cementless THA with the cancellous metal Lübeck hip system led to better longevity at 20 years than cemented THA with the Bioceram for patients with OA secondary to hip dysplasia

Introduction:. Most cases of hip osteoarthritis (OA) are believed to be caused by alterations in joint contact mechanics resulting from pathomorphologies such as acetabular dysplasia and acetabular retroversion. Over the past 13 years, our research group has focused on developing approaches for patient-specific modeling of cartilage and labrum in the human hip, and applying these approaches to study hip pathomorphology. The long term objective is to improve the understanding of the etiology of OA related to hip pathomorphology, and to improve diagnosis and treatment. The objectives of this presentation are to provide a summary of our subject-specific modeling approach, and to describe the results of our analysis of hips from three populations of subjects: normal, traditional dysplastic, and retroverted. Methods:. A combined experimental and computational protocol was used to investigate contact mechanics in ten normal subjects (normal center edge angles (CEA), no history of hip pain), ten subjects with hip pain secondary to acetabular dysplasia (CEA less than 25°), and ten patients with a radiographic crossover sign, pain and clinical exams consistent with acetabular retroversion. CT arthrography was used to image cartilage and bone. Volumetric image data were segmented and discretized, and subject-specific finite element models were produced using validated methods [Fig. 1]. Boundary and loading conditions were obtained from instrumented implant and gait data. Contact mechanics were evaluated on the acetabular cartilage and labrum. Labrum contact area and peak contact stress were evaluated. Cartilage contact area, peak and average contact stress were evaluated in six anatomical regions in the acetabulum. Results:. Hip contact patterns were subject-specific, but distinct patterns emerged in the groups. Dysplastic hips had a larger contact area in the lateral region of the acetabulum, while normal hips demonstrated a more distributed contact pattern. The labrum in dysplastic hips supported significantly more load than the labrum in normal hips in all activities [Fig. 2]. Contact in retroverted hips tended to be focused medially and superiorly [Fig. 3]. Retroverted subjects had smaller contact stress and area in most regions. Discussion:. The differences in labrum mechanics between the normal and dysplastic groups provide clear support for the mechanical importance of the acetabular labrum in dysplastic hips. There were only minor differences in cartilage contact stress and area between normal and dysplastic groups, because of a lateral shift in the location of contact and subsequent loading on the acetabular labra in the dysplastic hips. The larger labrum load support and contact area in dysplastic hips indicates that the labrum compensates for the shallow acetabula. Clinically, this may account for the pattern of OA onset in dysplastic hips. The results for the retroverted group do not support the commonly held belief that concentrated posterior loading in retroverted hips leads OA because there were lower contact stresses and areas in the posterior regions of retroverted hips. Further, these results suggest that rim trimming may be appropriate for retroverted hips. The preferred surgery likely depends on subtle patient specific aspects of hip pathoanotomy in both retroverted and dysplastic hips

Purpose. Femoroacetabular impingement (FAI) may contribute to the development of early onset hip osteoarthritis (OA). A cam lesion (or pistol grip deformity) of the proximal femur reduces head-neck offset resulting in cam type FAI. The alpha angle is a radiographic measurement recommended for diagnosis of cam type FAI. The purpose of this study was to determine if patients that develop end stage hip OA prior to 55 years of age have radiographic evidence of cam type FAI. Method. The anteroposterior (AP) pelvis and lateral hip radiographs of 244 patients (261 hips) who presented to our institution for hip arthroplasty or hip fracture fixation between 2006 and 2008 were retrospectively reviewed. Three cohorts were compared: 1) patients with end stage hip OA < 55 years old (N=76); 2) patients with end stage hip OA > 55 years old (N=84); 3) hip fracture patients > 65 years old without radiographic evidence of hip arthritis were used as controls (N=101). Patients with inflammatory arthritis, avascular necrosis and post-traumatic hip OA were excluded. Alpha angles were measured on the AP pelvis and lateral radiographs by three coauthors using ImageJ 1.43 software (National Institutes of Health, USA). For patients with end stage hip OA, AP alpha angles were measured on both the hip with OA and the contralateral hip. Lateral alpha angles were measured only on the hip with OA. For patients with hip fracture, AP alpha angles were measured on the non-fractured hip and lateral alpha angles were measured on the fractured hip. A one-way ANOVA with post hoc Tukeys HSD test was used to compare the AP and lateral alpha angles for the three cohorts. Results. The intraclass correlation coefficient (ICC) for the three coauthors measuring AP and lateral alpha angles was 0.85 and 0.86 respectively, indicating excellent inter-rater agreement. Patients < 55 years old with end stage hip OA had the largest AP and lateral alpha angles (82.711.6 degrees AP and 63.918.5 degrees lateral). These angles were significantly larger (p<0.01 for both comparisons) than patients > 55 years old with end stage hip OA (71.717.8 degrees AP and 55.518.0 degrees lateral) and hip fracture patients without hip OA (52.710.9 degrees AP and 44.411.4 degrees lateral). Comparing AP alpha angles of the contralateral hips, the mean AP alpha angle for patients < 55 years old with hip OA (70.813.2 degrees) was significantly larger (p=0.04) than patients > 55 years old with hip OA (64.516.2 degrees) which in turn was significantly larger (p<0.01) than the hip fracture patients (52.710.9 degrees). Conclusion. Patients < 55 years old with hip OA had the largest mean AP and lateral alpha angles, significantly larger than patients > 55 years old with hip OA and hip fracture patients without hip OA. Thus young patients with end stage hip OA do have radiographic evidence of cam type FAI. Furthermore, this case-controlled study suggests that cam type FAI may contribute to the development of early onset hip OA

Aims

The aetiologies of common degenerative spine, hip, and knee pathologies are still not completely understood. Mechanical theories have suggested that those diseases are related to sagittal pelvic morphology and spinopelvic-femoral dynamics. The link between the most widely used parameter for sagittal pelvic morphology, pelvic incidence (PI), and the onset of degenerative lumbar, hip, and knee pathologies has not been studied in a large-scale setting.

Methods

A total of 421 patients from the Cohort Hip and Cohort Knee (CHECK) database, a population-based observational cohort, with hip and knee complaints < 6 months, aged between 45 and 65 years old, and with lateral lumbar, hip, and knee radiographs available, were included. Sagittal spinopelvic parameters and pathologies (spondylolisthesis and degenerative disc disease (DDD)) were measured at eight-year follow-up and characteristics of hip and knee osteoarthritis (OA) at baseline and eight-year follow-up. Epidemiology of the degenerative disorders and clinical outcome scores (hip and knee pain and Western Ontario and McMaster Universities Osteoarthritis Index) were compared between low PI (< 50°), normal PI (50° to 60°), and high PI (> 60°) using generalized estimating equations.
