Introduction. Modern cementless press fit stems rely on early fixation and stability for osteointegration and longterm success with early migration increasing the risk of failure. The Ein Bild Roentegen Analyse Femoral Component Analysis (EBRA-FCA) methods allow accurate measurement of femoral stem subsidence without the need for Tantalum markers. The degree of subsidence of femoral stems in the first two years has been shown to be highly predictive of failure when using the cut off value of 1.5 mm. We aimed to measure the early migration pattern of a titanium alloy, tapered, plasma and hydroxyapatite coated femoral stem and any factors associated with subsidence. Methods. Between January 2005-June 2007, 387 Accolade cementless femoral stems (Stryker, Allendale NJ) were implanted at our institution. Seventy-seven had a minimum of two years post operative follow up and a complete set of pre and postoperative radiographs for analysis. Our group inlcuded 45 females with a mean age of 71.4 years, and 32 males with a mean age of 68.5 years. The primary diagnosis was degenerative osteoarthritis in 71 patients, avascular necrosis in two, and post fracture in four patients. The average BMI was 27.1. We measured the canal index to assess bone quality and the canal calcar index to assess the proximal femoral morphology. Immediate postoperative radiographs were assessed for canal fill of the prosthesis and implantation varus/valgus angles. The EBRA-FCA software was used to obtain migration curves for each stem. Results. The mean follow up was 29.3months (24–48). The mean canal index was 0.55 (0.36–0.68) with a mean canal calcar index of 0.54 (0.39–0.79). The average canal fill index at the midpoint of the stem was <0.8 in 37 stems and >80 in 40 stems. The average subsidence at 24 months was 2 mm and this had risen to 2.4 mm by 36 months postoperatively. When analysed using a Kaplan Meier curve using 1.5 mm as an end point we found a survivorship of 63.4% (52.3–74.5) at 24 months and this had worsened to 41.6% (26.6–56.5) by 36 months. Multivariate and univariate regression analysis of measured variables did not reveal any significant hazard for any factor other than the larger stem sizes doing worse. Discussion. Although several cementless tapered stem designs have had an excellent track record, our migration analysis of the Accolade stem is somewhat concerning. Thirty three percent of stems had reached the 1.5 mm subsidence point by two years. This is of concern as work has previously shown this to predict failure of stems with aseptic loosening at ten years with an accuracy of 79%. If these stems go on to fail at the predicted rate this would represent an unacceptably high level of failure. Our data raises serious concerns about the overall clinical performance of this stem design due to poor initial stability and integration

Introduction. Several short femoral stems have been introduced in primary total hip arthroplasty, supposedly in order to save proximal bone stock. We intended to analyse primary stability, changes in periprosthetic bone mineral density (BMD), and clinical outcome after insertion of the uncemented collum femoris preserving (CFP)-femoral device. Methods. A prospective cohort study on 30 patients scheduled for receiving the CFP-stem combined with an uncemented cup was carried out. Stem migration was analysed by radiostereometry (RSA). Preoperative total hip BMD and postoperative periprosthetic BMD in Gruen zones 1–7 was investigated by DXA, and the Harris hips score (HHS) was determined. The patients were followed up to 12 months. Results. 2 patients were intraoperatively excluded because their proximal femur was found to be unsuitable for insertion of the studied implant, 1 patient was later revised due to a deep infection. This left 27 patients for final analysis. RSA showed that only very little migration of the implant occurred, with the largest amplitude found in rotation around the y-axis (1.8°, SD 0.6, after 12 mths), representing minimal stem retroversion. DXA after 12 mths demonstrated substantial BMD loss in Gruen zones 7 (−30.8%), 6 (−19.1%) and 2 (−13.3%, p-values for all described changes <0.001 when comparing with baseline BMD determined immediately postoperatively). There was a moderate correlation of low preoperative total hip BMD with a higher amount of bone loss in Gruen zones 2 (Pearson correlation coefficient r = 0.6, p = 0.001), 6 (r = 0.5, p = 0.005) and 7 (r = 0.6, p = 0.003). In contrast, we found no correlation of periprosthetic bone loss in any of the Gruen zones 1–7 with logarithmically transformed maximal total point translation (MTPT) of the stem (p > 0.05 for all regions), neither after 3 nor after 12 mths. The mean HHS increased from 49 (SD 15) preoperatively to 99 (SD 2) after 12 mths. Interpretation. Based on these short-term data, we conclude that i) the studied implant seems to be stable within the first year, ii) substantial loss in periprosthetic BMD - with a predominance in the calcar region - occurs, iii) low preoperative total hip BMD predisposes towards greater loss of periprosthetic BMD after 12 months, iv) postoperative loss in periprosthetic BMD does not correlate with increased stem migration. Clinical results are excellent so far. Continuing follow-up will reveal whether this novel stem remains stable in the medium and long term, and whether the loss in BMD in the regions mentioned above can be recovered with time or whether it continues

The Olympia femoral stem is a stainless steel, anatomically shaped, polished and three-dimensionally tapered implant designed for use in cemented total hip arthroplasty (THA). The primary aim of this study was to determine the long-term survivorship, radiographic outcome, and patient reported outcome measures (PROMs) of the Olympia stem. Between May 2003 and December 2005, 239 patients (264 THAs) underwent a THA with an Olympia stem in our institution. PROMs were assessed using the Oxford Hip Score (OHS), EuroQol-5 dimensions (EQ-5D) score and patient satisfaction at mean 10-years following THA. Patient records and radiographs were then reviewed at a mean of 16.5 years (SD 0.7, 15.3 to 17.8) following THA to identify occurrence of complications or revision surgery for any cause. Mean patient age at surgery was 68.0 years (SD 10.9, 31–93 years). There were 156 women (65%, 176 THAs). Osteoarthritis was the indication for THA in 204 patients (85%). Stem survivorship at 10 years was 99.2% (95 % confidence interval [CI], 97.9%-100%) and at 15 years was 97.5% (94.6%–100%). The 15-year stem survival for aseptic loosening was 100%. Only one occurrence of peri-prosthetic fracture was identified, with no episodes of dislocation found. At a mean of 10 (SD 0.8, 8.7 –11.3) years follow-up, mean OHS was 39 (SD 10.3, range 7 – 48) and 94% of patients reported being very satisfied or satisfied. The Olympia stem demonstrated excellent 10-year PROMs, very high rates of stem survivorship and negligible peri-prosthetic fracture and dislocation rate at final follow-up beyond 15 years

To investigate predictors of periprosthetic fracture level (around stem (Vancouver B) or distal to stem (Vancouver C/D) in cemented polished tapered femoral stems. Retrospective cohort study of 188 patients (mean age 79 (range 30–91); 99 (53%) male) with unilateral periprosthetic femoral fractures associated with CPT stems. Medical notes were reviewed and the following recorded: patient demographics, past medical history, drug history, date of prosthesis insertion, and date of injury. Radiographs analysis included Vancouver classification, cement restrictor type, cement mantle to implant tip distance, cortical thickness, femoral diameter and DORR classification. Univariate, multivariate and ROC curve analysis was performed. Fractures occurred at mean 7.5 years following primary procedure: 152 (83%) were B fractures; and 36 (19%) C/D. On univariate analysis female gender, lower BMI, osteoporosis, NSAID use, Bisphosphonate therapy, cortical thickness, distal cement mantle length and distal cement mantle length:femoral diameter ratio were significantly associated with C level fractures (p<0.05). Distal cement mantle lengths of >19.6mm (AUC 0.688, p<0.001) were associate with C level fractures. Multivariate analysis demonstrated female gender and distal cement mantle length:femoral diameter ratio to be independent predictors of C level periprosthetic fractures. Though female sex is the largest independent predictor of periprosthetic fractures distal to a CPT femoral stem, the relationship between cortical thickness and distal cement mantle length appears significant. As fractures distal to the stem are invariably managed by ORIF, whereas fractures around the stem frequently require revision arthroplasty, this has relevance at primary surgery in osteoporotic females to reduce the need for complex revisions

Aims

Periprosthetic fractures (PPFs) following hip arthroplasty are complex injuries. This study evaluates patient demographic characteristics, management, outcomes, and risk factors associated with PPF subtypes over a decade.

Methods

Using a multicentre collaborative study design, independent of registry data, we identified adults from 29 centres with PPFs around the hip between January 2010 and December 2019. Radiographs were assessed for the Unified Classification System (UCS) grade. Patient and injury characteristics, management, and outcomes were compared between UCS grades. A multinomial logistic regression was performed to estimate relative risk ratios (RRR) of variables on UCS grade.

Total hip replacements have been shown to give superior outcomes in patients with intracapsular fractures of the neck of femur compared with hemiarthroplasty. Collarless uncemented femoral stems give excellent long term results in elective hip replacements but there are few studies looking at their outcomes in fractured neck of femur patients. There is some concern that in trauma patients bone quality maybe inferior as most neck of femur fractures are secondary to osteoporosis. The presence of osteoporosis and subsequent widened femoral canal may compromise the mechanical stability of uncemented femoral stems and result in early subsidence, which can lead to altered leg length and decreased hip stability. The aim of this study was to assess whether early subsidence occurred when collarless uncemented stems were used to treat patients with fractures of the neck of femur. Post-operative radiographs of 33 patients, mean age 71, who underwent an uncemented collarless total hip replacement for a fracture, were reviewed. The distance from the calcar to the tip of the prosthesis was measured for each patient on the initial post operative radiograph and again on the follow-up radiograph at 6 months post operation. Any subsidence was recorded and magnification for each radiograph was calculated by measuring the diameter of the femoral head, which was known to be 36mm. Distances were then adjusted for magnification. The same procedure was performed on 36 age-matched patients, mean age 71, who underwent elective uncemented total hip replacements for osteoarthritis. Hospital notes for each patient were reviewed to assess for complications and DEXA scan results for trauma patients were also evaluated where available. The mean femoral stem subsidence was significantly greater in the fracture cohort than in elective patients (p = 0.001) with mean subsidence of 4.07mm (range 0.02–18.5mm) and 1.57mm (range 0–5.5mm) respectively. In the fracture cohort there were 3 revisions within 6 months of surgery, 1 for infection and 2 for femoral stem subsidence leading to dislocation. There were no revisions in the elective cohort. DEXA scan results were available for 21 of the 33 fracture cohort patients. All these patients had abnormal bone density with 52% being osteoporotic and 48% osteopenic. This study showed that collarless uncemented stems subsided significantly when performed for fractures and had a high early revision rate. We therefore recommend that cemented or collared femoral stems be used in patients with femoral neck fractures requiring total hip replacement to reduce the risk of femoral stem subsidence

Aims

To evaluate if, for orthopaedic trainees, additional cadaveric simulation training or standard training alone yields superior radiological and clinical outcomes in patients undergoing dynamic hip screw (DHS) fixation or hemiarthroplasty for hip fracture.

Methods

This was a preliminary, pragmatic, multicentre, parallel group randomized controlled trial in nine secondary and tertiary NHS hospitals in England. Researchers were blinded to group allocation. Overall, 40 trainees in the West Midlands were eligible: 33 agreed to take part and were randomized, five withdrew after randomization, 13 were allocated cadaveric training, and 15 were allocated standard training. The intervention was an additional two-day cadaveric simulation course. The control group received standard on-the-job training. Primary outcome was implant position on the postoperative radiograph: tip-apex distance (mm) (DHS) and leg length discrepancy (mm) (hemiarthroplasty). Secondary clinical outcomes were procedure time, length of hospital stay, acute postoperative complication rate, and 12-month mortality. Procedure-specific secondary outcomes were intraoperative radiation dose (for DHS) and postoperative blood transfusion requirement (hemiarthroplasty).

Aims

Despite multiple trials and case series on hip hemiarthroplasty designs, guidance is still lacking on which implant to use. One particularly deficient area is long-term outcomes. We present over 1,000 consecutive cemented Thompson’s hemiarthroplasties over a ten-year period, recording all accessible patient and implant outcomes.

Methods

Patient identifiers for a consecutive cohort treated between 1 January 2003 and 31 December 2011 were linked to radiographs, surgical notes, clinic letters, and mortality data from a national dataset. This allowed charting of their postoperative course, complications, readmissions, returns to theatre, revisions, and deaths. We also identified all postoperative attendances at the Emergency and Outpatient Departments, and recorded any subsequent skeletal injuries.

Aims

The aim of this study was to describe the management and associated outcomes of patients sustaining a femoral hip periprosthetic fracture (PPF) in the UK population.

Methods

This was a multicentre retrospective cohort study including adult patients who presented to 27 NHS hospitals with 539 new PPFs between 1 January 2018 and 31 December 2018. Data collected included: management strategy (operative and nonoperative), length of stay, discharge destination, and details of post-treatment outcomes (reoperation, readmission, and 30-day and 12-month mortality). Descriptive analysis by fracture type was performed, and predictors of PPF management and outcomes were assessed using mixed-effects logistic regression.

Introduction. Malalignment of some designs of stem is associated with an increased risk of aseptic loosening and revision. We investigated whether the alignment of the cemented polished, double-taper design adversely affected outcome, in a multicentre prospective study. Methods. A multicentre prospective study of 1189 total hip replacements was undertaken to investigate whether there is an association between surgical outcome and femoral stem alignment. All patients underwent a primary THR with the Exeter femoral stem (Stryker Howmedica Osteonics, Mahwah, NJ) and a variety of acetabular components. The primary outcome measure was the Oxford hip score (OHS) and change in OHS at five years. Secondary outcomes included rate of dislocation and revision. Radiographic evaluation of the femoral component was also undertaken. The long axis of the Exeter femoral component and the long axis of the femoral canal were located, and the angle at the point of intersection measured. The cementing quality was determined as defined by Barrack et al. Radiolucent lines at the cement-stem and cement-bone interface in the five year radiographs were defined using the zones described by Gruen et al. Subsidence was measured as the vertical dimension of the radiolucency craniolateral to the shoulder of the stem in Gruen zone 1 as described by Fowler et al. Cement fractures were recorded. Results. The incidence of varus (>5 ° to the femoral axis), and valgus (>5 °) malignment were 3.7% and 0.8% respectively. Pre-operative demographics and OHS were similar in all groups (p > 0.4). There was no significant difference in OHS or change in OHS between neutral and malaligned groups at 5 years (neutral, mean=40.1, change=23.1; varus, mean=40.1, change=23.7; valgus, mean=42.0, change=26.6; p=0.46 and p=0.45 respectively). There was no significant difference in dislocation rate between the groups (p=0.66). There was also no significant difference in revision rate (p=0.34). There were no statistically significant differences in the incidence of femoral radiolucency, stem subsidence or cement fracture (p > 0.1). Conclusion. This study provides evidence that both varus and valgus implantation does not compromise the short to medium term clinical results of the cemented, polished, double-taper stems. Longer follow-up is required to establish the influence of stem alignment on the incidence of aseptic loosening and revision

Revision hip surgery is reportedly rising inexorably yet not all units report this phenomenon. The outcome of 1143 consecutive Corin TaperFit primary hip arthroplasties (957 patients) performed between 1995 and 2010 is presented. The implants were cemented under pressurisation and combined the TaperFit stem with Ogee flanged cups. Data was gathered from local arthroplasty database and case note review of revised joints. 13 hips have been revised (1.1%). Cumulative prosthesis survival is 0.99 +/− 0.0. Two femoral stems were revised (0.2%); one at 6 months for sepsis, one at 14 days after dislodgment during reduction of dislocation. No revisions were undertaken for aseptic loosening of the stem or cup, nor for thigh pain. 32 patients (32 hips) ≥15 year follow up, 13 survive today and none have been revised (0%). Of the 471 with ≥10 year follow up, 38 were aged ≤50 at time of surgery and 1/38 has been revised to date (PLAD for dislocation). The strong population stability in this region, supported by independent investigation by Scottish Arthroplasty Project, endorses the accuracy of the data quoted. The low incidence of revision in this cohort, and absence of revision for aseptic loosening (mean follow up 8.03 years +/− SD 3.94; range 18 months to 16yrs 2 months), substantially supports the longevity and use of cemented, double-taper, polished, collarless femoral stems in combination with cemented polyethylene cups in primary hip arthroplasty in all patient age groups

Aim. to compare the medium term clinical and functional results of total hip arthroplasty after intertrochanteric osteotomy and primary total hip replacement. Material and methods. We compared 2 groups of patients: Group I-93 patients with total hip arthroplasty after intertrochanteric osteotomy and Group II-93 patients with primary total hip arthroplasty. The patients in the control group (Group II) were randomly chosen from the cases operated by the same surgical team using the same kind of implant like those in Group I. The osteotomies were of different types: medial displacement (27), varisation(19), valgisation (18), flexion(25), rotational (4). The demographic parameters were similar in the 2 groups. Technical intraoperative challenges were noted. The Harris Score was determined at 12 months and 3 years after surgery. Clinical and radiological check-ups were performed each year. The duration of each surgical procedure was recorded. Complications were noted during the entire follow-up. The mean follow-up was 77 months. Results. The average time interval between osteotomy and total hip replacement was 76 months. The average Harris Score (HHS) at 1 year after surgery was 88 in Group I and 93 in Group II. At 3 years, the average values of HHS were 85 in the first group and 92 in the second. After an average follow-up of 6 years, 6 patients in group I (6,45%) required revision arthroplasty, while in the second group this procedure was not necessary. The indication of revision was loosening of the femoral component in 4 cases and of both components in 2 cases. The duration of a total hip arthroplasty after an ostheotomy was almost two times longer than a primary total hip replacement. In most cases, the surgical procedure implicated removal of a blade plate or others fixation devices, reaming of the femur, osteotomies of the greater trochanter, reverse osteotomies of the intertrochanteric region and the use of special design (thin or curved) femoral stems. In group I, intraoperative complications included fractures of the femoral diaphysis, fractures of the greater trochanter, false passage for the femoral component and malposition of the femoral stem. Postoperative complications consist in dislocations, early loosening, heterotopical ossifications, wound sepsis, etc. Conclusions. Total hip arthroplasties after intertrochanteric osteotomies have clinical results a little bit inferior of those of primary total hip replacements, but the difference is not significative. The survival rate of prosthesis implanted after intertrochanteric osteotomies at 6 years is significantly inferior. Both the intraoperative and postoperative complications rates are far superior of those of the primary arthroplasties

National Institute of Clinical Excellence guidelines state that cemented stems with an Orthopaedic Data Evaluation Panel (ODEP) rating of > 3B should be used for hemiarthroplasty when treating an intracapsular fracture of the femoral neck. These recommendations are based on studies in which most, if not all stems, did not hold such a rating. . This case-control study compared the outcome of hemiarthroplasty using a cemented (Exeter) or uncemented (Corail) femoral stem. These are the two prostheses most commonly used in hip arthroplasty in the UK. Data were obtained from two centres; most patients had undergone hemiarthroplasty using a cemented Exeter stem (n = 292/412). Patients were matched for all factors that have been shown to influence mortality after an intracapsular fracture of the neck of the femur. Outcome measures included: complications, re-operations and mortality rates at two, seven, 30 and 365 days post-operatively. Comparable outcomes for the two stems were seen. . There were more intra-operative complications in the uncemented group (13% vs 0%), but the cemented group had a greater mortality in the early post-operative period (n = 6). There was no overall difference in the rate of re-operation (5%) or death (365 days: 26%) between the two groups at any time post-operatively. This study therefore supports the use of both cemented and uncemented stems of proven design, with an ODEP rating of 10A, in patients with an intracapsular fracture of the neck of the femur. Introduction. Total Hip Arthroplasty (THA) in patients after proximal femoral osteotomy remains a major challenge. Inferior survival for both cementless and cemented THA has been reported in this subgroup of patients. Methods. We retrospectively evaluated the clinical and radiographic results of a consecutive series of 48 THAs (45 Patients) who had undergone conversion THA for failed intertrochanteric osteotomy after a mean of 12 years (2–33 years) using a cementless, grit-blasted, double-tapered femoral stem. Mean follow-up was 20 years (range, 15–25 years), mean age at surgery was 47 years (range, 13–55 years). Clinical results were evaluated using the Harris Hip Score. Kaplan-Meier survivorship analysis was performed to determine long-term outcomes for different end points. Results. At follow-up, 10 patients (11 hips) had died, and 1 patient (1 hip) was lost to follow-up. Five patients (5 hips) underwent femoral revision, 2 for infection and 3 for aseptic loosening of the stem. Overall stem survival was 91% at 20 years (95%-CI: 78%–97%); survival with femoral revision for aseptic loosening as end point was 93% (95%-CI: 80%–98%). Discussion and Conclusion. The long-term results with this type of cementless femoral component in young patients with failed intertrochanteric osteotomy are encouraging and compare well to those achieved in patients with regular femoral anatomy

Introduction. There has been renewed interest in the unicompartmental knee arthroplasty with reports of good long term outcomes. Advantages over a more extensive knee replacement include: preservation of bone stock, retention of both cruciate ligaments, preservation of other compartments and better knee kinematics. However, a number of authors have commented on the problem of osseous defects requiring technically difficult revision surgery. Furthermore, a number of recent national register studies have shown inferior survivorship when compared to total knee arthroplasty. The purpose of this study was to review the cases of our patients who had a revision total knee arthroplasty for failed unicompartmental knee arthroplasty. To determine the reason for failure, describe the technical difficulties during revision surgery and record the clinical outcomes of the revision arthroplasties. Methods. Between 2003 and 2009 our institute performed thirty three revisions of a unicompartmental knee arthroplasty on thirty two patients. The time to revision surgery ranged from 2 months to 159 months with a median of 19 months. Details of the operations and complications were taken form case notes. Patient assessment included range of motion, need for walking aids and the functional status of the affected knee in the form of the Oxford knee score questionnaire. Results. The reasons for failure were aseptic loosening of tibial component, persistent pain, dislocated meniscus, mal-alignment and osteoarthritis in another compartment. Of the 33 revision knee arthroplasties 18 required additional intra-operative constructs. 11 knees required a long tibial stem while 1 required a long femoral stem. 10 knees required medial wedge augmentation and bone graft was used in 6. Mean 1 year Oxford knee scores for failed unicompartmental knee replacements was 29 compared to 39 for primary total knee replacements performed at the same institute. Of the revision knee replacements 2 required further revision due to infection and loosening. Conclusion. From the evidence of our group of failed unicompartmental knee replacements, revision surgery is technically difficult and often requires intra-operative constructs. Clinical outcome of revision total knee arthroplasty following failed unicompartmental knee arthroplasty is not comparable to primary total knee arthroplasty

Background. Wear particles are considered to be the major culprit for the aseptic loosening. Their characterization is thus crucial for the understanding of their bioreactivity and contribution to the development of aseptic loosening. Methods. Metal wear debris particles were analyzed directly in periprosthetic tissue resins by scanning electron microscopy (SEM) combined with back-scattered electron imaging (BSE) and energy dispersive X-ray spectroscopy (EDS). Four groups of tissue samples retrieved at revision operations of loosened hip implants with different bearing surfaces (metal-on-metal, ceramic-on-polyethylene and metal-on-polyethylene), and different material of the femoral stem (Ti alloy, CoCrMo and polymer combined with stainless steel) were investigated. Tissue samples were first analyzed histologicaly. Sections from the same paraffin blocks were then carbon coated and analyzed using SEM/BSE/EDS method. Results. Metal particles were detected in all samples. Their composition corresponded to the composition of the implant components. The gradation of metal particles ranged from +1 to +3. A considerable number of big metal particles were actually agglomerates of submicron particles visible only at higher magnification. The clustering of particles was observed primarily for CoCrMo and, to a lesser extent, for stainless steels particles. The median sizes of CoCrMo clusters in two groups of samples were 2.9 1.8 m (range, 0.5 to 7.6 m) and 3.2 1.0 m (range, 1.9 to 5.4 m). The effect of clustering was not observed for Ti particles. The median sizes of individual Ti particles determined in two groups of samples were 2.5 3.6 m (range, 0.4 to 17.3 m) and 4.3 2.8 m (range, 0.8 to 11.0 m). Conclusion. Scanning electron microscopy combined with back-scattered electron imaging is an appropriate and selective method to recognize metal particles in tissue sections, without being destructive to specimens. When the size of the particles is considered, however, it should be differed between the size of individual particles and size of clusters of particles. Besides its benefits, this study has some limitations: the detection of particles smaller than 0.4 m is difficult, and this method cannot be used to identify polyethylene particles

Introduction. Total femur implantation is a rare and challenging procedure in final revision surgery of hip and knee arthroplasties. Reports of this operation technique are even rare in literature. In this study we retrospectively analyse our patients with total femur implants. Material and Methods. Between October 2002 and February 2009 we implanted 27 total femurs in hip and knee revision surgery cases. We used the modular prosthesis system–Megasystem C® by Waldemar Link–in all cases. Our 22 female and 5 male patients had a mean age of 76 years (range 45–88). Indications for the procedure were loosening of megaprosthesis of the hip in 12 cases; 2 with massive distal migration, 1 with penetration into the knee joint and 1 after two step revision procedure. In 13 cases a periprosthetic indicated a total femur implanatation due to massive bone loss; 1 breakage of a long femoral stem, 5 fracture of osteosynthesis materials, 3 after prosthesis revisions and 1 pseudoarhrosis of femur and tibia after knee arthroplasty. We analysed perioperative complications, clinical status and result and further revision within follow up time. Results. We were able to examine 16 patients at follow up time. Eight patients were lost to follow up and 2 have been died; 1 after fulminant pulmonal embolia after operation and 1 four years postoperatively. One total femur had to be exchanged due to infection after 1 year. Perioperative complications occurred as follows: 1 massive blood transfusion, 1 peroneal palsy, 1 ulcus ventriculis bleeding, 1 thrombosis of vena suclavia and vena jugularis, 1 sigmaresection due to diverticulosis, 1 luxations of the hip, and 2 wound necrosis. In 3 caese a revision operation swas performed; 1multiple luxtion of the hip and due to infection of the total femur implant. The range of motion of the hip was 85 degrees (range 30–90) and knee 92 degrees (range 30–110). In nearly all cases we found a lengthening of the revised limb. The general outcome of the patients was. Ten patient were very satisfied, 2 statisfied and 2 fair due to pain persistence. Two patient were mobile with one crutch, 3 used two crutches and one was able to walk with a rollator. One patient was unable to walk due to diplegia after spine fracture. Conclusion. Total femur procedure in final revision arthroplasty has a high potential of perioperative risks but has shown good clinical and mobility results in our patient group. With the Megasystem C® by Link we had a save and good performance while operation of this difficult patient group

Introduction. treatment of femoral neck fractures in the physically active elderly, is still an open question. The comparison of total replacement and partial hip replacement showed substantial superiority of the first approach than the latter. We can not disregard the problems that THR can lead in the elderly: more surgical time, greater blood loss, major sacrifice of bone in osteoporotic subjects. The Tribofit® acetabular system, has the objective to overcome these problems with the implantation of a single 3mm soft, pliable buffer made of polycarbonate-urethane between the large diameter metal femoral head and the subchondral bone, thus replacing the articular cartilage. The surgical technique includes the reaming of the cartilage layer, the creation of a circumferential groove and the snap-fit insertion of the buffer. The purpose of the prospective study was to evaluate the effectiveness of this approach in the treatment of femoral neck fractures in the elderly. Materials and Methods. from September 2008 to July 2010 we performed 45 implants with the Tribofit® Acetabular Buffer (Active Implants®, Memphis TN, USA). The inclusion criteria for the study were: femoral neck fracture patients over 75 years of age, physically independent before the injury, with good life expectancy, mild or absent arthritis, absence of morphological alterations of the acetabulum. The average age of these patients was 81,2 (75–89), the sex distribution was 36 females and 9 males, with 22 fractures of the left hip and 23 of the right. In all cases an uncemented straight femoral stem was implanted. We recorded per-operative blood loss and intra-per-operative problems encountered. Follow-up were performed at 1, 3, 6 and 12 months post-op with radiographic control and clinical examination. The clinical outcome was objectively measured by the Harris Hip Score, pain Visual Analogue Scale and SF-36 questionnaire. Results. Of the 45 patients, 4 died of unrelated causes and 3 were lost to follow-up (2 due to cognitive impairment and 1 due to peri-prosthetic fracture of the femur). Of the 38 available for follow-up, all were followed at the staged time. No intra-operative problems were registered. The mean HHS for these fractures patients was 74,2 at 6 months and 86 at 12 months (90–80 good; 80–70 fair). The mean pain VAS was 4,3 at 6 months and 2 at 12 months. The mean SF-36 Score was 52,3 at 6 months and 53,2 at 12 months. Of the 38 followed, we had 2 dislocations in the third week post-op while the two patients were in a rehabilitation centre. Open reduction was carried out and in both cases the buffer was not dislodged. Discussion and conclusions. we believe the results with this new type of prosthesis are encouraging and comparable to if not better than other prosthetic options in the literature for this difficult-to-treat patient population

Aims

Currently, periprosthetic fractures are excluded from the American Society for Bone and Mineral Research (ASBMR) definition of atypical femoral fracture (AFFs). This study aims to report on a series of periprosthetic femoral fractures (PFFs) that otherwise meet the criteria for AFFs. Secondary aims were to identify predictors of periprosthetic atypical femoral fractures (PAFFs) and quantify the complications of treatment.

Patients and Methods

This was a retrospective case control study of consecutive patients with periprosthetic femoral fractures between 2007 and 2017. Two observers identified 16 PAFF cases (mean age 73.9 years (44 to 88), 14 female patients) and 17 typical periprosthetic fractures in patients on bisphosphonate therapy as controls (mean age 80.7 years (60 to 86, 13 female patients). Univariate and multivariate analysis was performed to identify predictors of PAFF. Management and complications were recorded.

Aims

Periprosthetic femoral fractures (PFF) following total hip arthroplasty (THA) are devastating complications that are associated with functional limitations and increased overall mortality. Although cementless implants have been associated with an increased risk of PFF, the precise contribution of implant geometry and design on the risk of both intra-operative and post-operative PFF remains poorly investigated. A systematic review was performed to aggregate all of the PFF literature with specific attention to the femoral implant used.

Patients and Methods

A systematic search strategy of several journal databases and recent proceedings from the American Academy of Orthopaedic Surgeons was performed. Clinical articles were included for analysis if sufficient implant description was provided. All articles were reviewed by two reviewers. A review of fundamental investigations of implant load-to-failure was performed, with the intent of identifying similar conclusions from the clinical and fundamental literature.
