Introduction. Snapping hip syndrome is a common condition affecting 10% of the population. It is due to the advance of the iliotibial band (ITB) over the greater trochanter during lower limb movements and often associated with hip overuse, such as in athletic activities. Management is commonly conservative with physiotherapy or can be surgical to release the ITB. Here we carry out a systematic review into published surgical management and present a case report on an overlooked cause of paediatric snapping hip syndrome. Materials & Methods. A systematic review looking at published surgical management of snapping hip was performed according to PRISMA guidelines. PubMed, MEDLINE, EMBASE, CINAHL and the Cochrane Library databases were searched for “((Snapping hip OR Iliotibial band syndrome OR ITB syndrome) AND (Management OR treatment))”. Adult and paediatric published studies were included as few results were found on paediatric snapping hip alone. Results. 1548 studies were screened by 2 independent reviewers. 8 studies were included with a total of 134 cases, with an age range of 14–71 years. Surgical management ranged from arthroscopic, open or ultrasound guided release of the ITB, as well as gluteal muscle releases. Common outcome measures showed statistically significant improvement pre- and post-operatively in visual analogue pain score (VAPS) and the Harris Hip Score (HHS). VAPS improved from an average of 6.77 to 0.3 (t-test p value <0.0001) and the HHS improved from an average of 62.6 to 89.4 (t-test p value <0.0001). Conclusions. Although good surgical outcomes have been reported, no study has reported on the effect of rotational profile of the lower limbs and snapping hip syndrome. We present the case of a 13-year-old female with snapping hip syndrome and trochanteric pain. Ultrasound confirmed external snapping hip with normal soft tissue morphology and radiographs confirmed no structural abnormalities. Following extensive physiotherapy and little improvement, she presented again aged 17 with concurrent anterior knee pain, patella mal-tracking and an asymmetrical out-toeing gait. CT rotational profile showed 2° of femoral neck retroversion and excessive external tibial torsion of 52°. Consequently, during her gait cycle, in order to correct her increased foot progression angle, the hip has to internally rotate approximately 35–40°, putting the greater trochanter in an anterolateral position in stance phase. This causes the ITB to snap over her abnormally positioned greater trochanter. Therefore, to correct rotational limb alignment, a proximal tibial de-rotation osteotomy was performed with 25° internal rotation correction. Post-operatively the patient recovered well, HHS score improved from 52.5 to 93.75 and her snapping hip has resolved. This study highlights the importance of relevant assessment and investigation of lower limb rotational profile when exploring causes of external snapping hip, especially where ultrasound and radiographs show no significant pathology

Introduction. A common acute orthopaedic presentation is an ulcerated or infected foot secondary to diabetic neuropathy. Surgical debridement or amputation are often required to manage this complication of diabetes. International literature indicates that amputation may lead to further complications and an increased mortality rate. The aim of this study is to investigate the mortality rate associated with different surgical interventions. This will inform surgical management of patients presenting with acute foot complications from diabetes. Methods. This is a retrospective review of patients with diabetic foot infections aged >16 years attending Middlemore Hospital over a 10-year period (2012–2021). Clinical records were examined to determine whether patients were managed with no surgery, surgery but not amputation, or amputation. We recorded relevant baseline characteristics and comorbidities. Regression models were used to determine factors associated with mortality. Results. Over the study period, 1260 patients were included in analysis. Patients were divided into three groups, a control group who received no surgical intervention (n=554), those receiving surgery but not amputation (n=269), and those who underwent amputation (n=437). After adjustment for potential confounders, mortality rates were significantly higher in those who underwent amputation compared with those who received surgical intervention without amputation. Survival probability at 1 year and 5 years was highest in the surgical intervention but not amputation group. Conclusion. It is clinically important that there is a lower mortality rate in patients who undergo surgical intervention without amputation. Treatment that aims to salvage the limb rather than amputate should be considered in management of patients with diabetic foot complications to optimise their care

Abstract. Background. Multi-ligament knee injury is a rare but severe injury. Treatment strategies are challenging for most orthopedic surgeons & optimal treatment remains controversial. The purpose of our study was to assess clinico-radiological and functional outcomes after surgical management of multi-ligament knee injuries & to determine factors that could predict outcome of surgery. Materials And Method. It is a prospective observational study of 30 consecutive patients of Multi-ligament knee injury conducted between 2018–2020. All patients were treated surgically with single-stage reconstruction of all injured ligaments and followed standardized postoperative rehabilitation protocol. All patients were evaluated for Clinical (VAS score, laxity stress test, muscle-strength, range of motion), Radiological (stress radiographs) & Functional (Lysholm score) outcomes three times-preoperatively, post-operative 3 & 12 months. Results. At final follow up mean VAS score was 0.86±0.77. The anteroposterior & valgus-varus stress test showed ligament laxity >10mm (GradeD) in 93.3% patient which improved to <3mm (normal, GradeA) in 90% patients. Most patients (83.3%) had preoperative-range <100° and muscle strength of MRC Grade-3 which improved to >120° and muscle strength of MRC grade-5 at final followup. Lysholm score was poor (<64) in all patients preoperatively and improved to good (85–94) in 73.3%, excellent (>95) in 20% & fair (65–84) in 6.6% patients. The stress radiographs showed stable results for anterior/posterior & varus/valgus stress. All patients returned to their previous work. Factors that could predict outcomes of surgery are age, timing of surgery, type of surgery & associated injury. Conclusion. Early complete single stage reconstruction can achieve good functional results with overall restoration of sports & working capacity. Positive predictive factors for good outcome are younger age, early surgery & appropriate rehabilitation

Sacral fractures are often underdiagnosed, but are frequent in the setting of pelvic ring injuries. They are mostly caused by high velocity injuries or they can be pathological in aetiology. We sought to assess the clinical outcomes of the surgically treated unstable sacral fractures, with or without neurological deficits. unstable sacral fractures were included in the study.

Single centre, prospectively collected data, retrospective review of patients who sustained vertically unstable fractures of the sacrum who underwent surgical fixation. out of a total of 432 patients with pelvis and acetabulum injuries. fifty six patients met the inclusion criteria. 18 patients had sustained zone one injuries. 14 patients had zone 2 injuries and 10 patients had zone 3 injurie. Operative fixation was performed percutaneously using cannulated screws in 18 patients.. Open fixation of the sacrum using the anterior approach in 6 patients. Posterior approach was indicates in all 10 of the zone 3 injuries of the sacrum. While in 4 patients, combined approaches were used. 3 patients had decompression and spinopelvic fixation.

Neurological deficits were present in 16% of the patients. 2 patients presented with neurgenic bladder. Of the 4 patients who had neurological fall out, 3 resolved with posterior decompression and posterior fixation. All 4 neurological deficits were due to taction or compression of the nerve roots. No hardware failures or non unions observed.

The rate of neurological deficit was related more to the degree of pelvic ring instability than to a particular fracture pattern. Low rates of complications and successful surgical treatment of sacral fractures is achiavable. Timeous accurate diagnosis mandatory

Aim. Skeletal tuberculosis (TB) accounts for up to one third of cases of extra-pulmonary TB but comprises a minority of osteoarticular infection in areas with low TB incidence. Consequently, unexpected cases may receive surgical management targeted at non-tuberculous orthopaedic infections. This study reviewed treatment and outcomes of non-spinal osteoarticular TB to assess outcomes from modern surgical techniques. Method. All patients with a diagnosis of non-spinal osteoarticular TB between 2009–2017 from one tertiary referral centre were included. Retrospective review of surgical intervention, antibiotic treatment and outcome was conducted. Results. Fourteen patients with an average age of 48 (range 20–77) were identified; all were HIV-negative. Articular infections affected 7 patients, including one prosthetic joint infection. Osteomyelitis affecting the carpus, femur, tibia, olecranon and metatarsals was diagnosed in the remaining patients. Only 4 patients had radiological findings consistent with prior pulmonary TB, and only 3 had a history of prior TB or TB exposure. In 2 cases, symptom exacerbation was associated with local steroid injection. Diagnostic biopsy was employed in 5 cases, of whom 4 proceeded to medical management. Diagnosis was made following positive culture in 86% of cases; all TB isolates were fully sensitive. 71% of cases underwent surgical treatment according to best practice for biofilm-forming infection, including excision of osteomyelitis with local antibiotic therapy for three patients, and first-stage excision with spacer implantation for four patients. Quadruple therapy for an average of 8.5 months, range 6–12 months, was administered. Patients were followed up for a mean of 15.2 months. Half of the patients treated with surgery reported ongoing pain at 3 months and 4 patients underwent further surgery for persistent signs of infection (2 for probable persistent TB, 2 for bacterial super-infection). Conclusions. The role of surgical debridement in management of osteoarticular TB is unclear. In patients with a previous history of TB exposure a pre-operative diagnosis of TB could prevent unnecessary surgery and therefore prevent associated post-operative complications including bacterial super-infection and pain. Pre-op biopsy should therefore be considered in all patients with a history of TB exposure

Proximal humerus fractures (PHF) are common, accounting for approximately 5% of all fractures. Approximately 30% require surgical intervention which can range from open reduction with internal fixation (ORIF) to shoulder arthroplasty (including hemiarthroplasty, total shoulder arthroplasty, (TSA) or reverse total shoulder arthroplasty (RTSA)). The aim of this study was to assess trends in operative interventions for PHF in an Australian population.

Data was retrospectively collected for patients diagnosed with a PHF and requiring surgical intervention between January 2001 and December 2020. Data for patients undergoing ORIF were extracted from the Medicare database, while data for patients receiving arthroplasty for PHF were obtained from the Australian Orthopaedic Association National Joint Replacement Registry (AOANJRR).

Across the study period, ORIF was the most common surgical procedure for management of PHFs. However, since 2019, RTSA has surpassed ORIF as the most common surgical procedure to treat PHFs, accounting for 51% of operations. While the number of RTSA procedures for PHF has increased, ORIF and shoulder hemiarthroplasty has significantly reduced since 2007 (p < 0.001). TSA has remained uncommon across the follow-up period, accounting for less than 1% of all operations. Patients younger than 65 years were more likely to receive ORIF, while those aged 65 years or greater were more likely to receive hemiarthroplasty or RTSA.

While the number of ORIF procedures has increased during the period of interest, it has diminished as a proportion of overall procedure volume. RTSA is becoming increasingly popular, with decreasing utilization of hemiarthroplasty, and TSA for fracture remaining uncommon. These trends provide information that can be used to guide resource allocation and health provision in the future. A comparison to similar data from other nations would be useful.

Atypical femoral fracture non-union (AFFNU) is both, rare (3–5 per 1000 proximal femur fractures) and difficult to treat. Lack of standardised guidelines leads to a variability in fixation constructs, use of bone grafting and restricted weight bearing protocols, which are not evidence based. We hypothesised that there is no change in union rates without the use of bone grafting and immediate weight bearing post-operatively does not lead to increased complications.

Materials & Methods

A retrospective review of all consecutively treated AFFNU cases between March 2015 to December 2019 was carried out. 9 patients with a mean age of 63.87 years and M:F ratio of 7:2 met the inclusion criteria. Primary outcome variable was radiographic union at 12 months after revision surgery. All surgeries were carried out by a single surgeon. Fixation construct, neck-shaft angle, use of bone graft and immediate postoperative weight bearing protocols were recorded.

Results

Radiographic union was achieved in 7 of 9 patients (78%) after first revision surgery. 1 patient achieved union after 2nd revision surgery and 1 patient died in the early post-operative period due to pulmonary embolism. No bone grafting was used in any of the patients and weight-bearing as tolerated was allowed from the first post-operative day. The mean neck-shaft angle after non-union surgery was 136 degrees.

Purpose of the study. The aim of the study was to review the surgical management of Neuromuscular Scoliosis (NMS) and its outcome. Method. NMS patients who underwent surgery between 2001 and 2012 were identified. Case notes, surgical data, complications and radiographic results were reviewed retrospectively. The families were contacted telephonically. Sitting posture status and whether the parents and patients would opt to have the surgery again if given the opportunity, was assessed. Summary of Results. 96 NMS patients had corrective spinal surgery performed by a single surgeon in 4 institutions. Cerebral Palsy was the most common underlying cause (18) followed by Myelomeningocoele (15), SMA (12) and post spinal cord injury scoliosis (10). 52 patients were female and 44 male with a median age of 13.6 years at surgery. The delay to surgery after presentation was 20.6 months. Seventy patients received posterior only based surgery and 12 patients had anterior only fusions. Another 12 patients had a 2-stage procedure with a delay of 12 days between operations. The average blood loss was 1600ml and duration of surgery 222 minutes. Forty seven patients had pedicle screw only constructs and 46 patients' constructs included the pelvis. An average of 14.2 levels were included in the construct. The primary curve was corrected from 84.4° to 32.7° which was mostly maintained throughout the follow-up period. 55 patients had a follow-up of greater or equal to 2 years. Only one patient reported not having improved his sitting posture. 3 parents and 5 patients would opt not to have the surgery again. Conclusion. Spine fusion in NMS is effective in correcting and maintaining correction of associated scoliosis despite the risk of major complications. Most patients and family value the clinical improvement and would agree to surgery again. NO DISCLOSURES

Ligament reconstruction following multi-ligamentous knee injuries involves graft fixation in bone tunnels using interference screws (IS) or cortical suspensory systems. Risks of IS fixation include graft laceration, cortical fractures, prominent hardware, and inability to adjust tensioning once secured. Closed loop suspensory (CLS) fixation offers an alternative with fewer graft failures and improved graft-to-tunnel incorporation. However, graft tensioning cannot be modified to accommodate errors in tunnel length evaluation. Adjustable loop suspensory (ALS) devices (i.e., Smith & Nephew Ultrabutton) address these concerns and also offer the ability to sequentially tighten each graft, as needed. However, ALS devices may lead to increased graft displacement compared to CLS devices. Therefore, this study aims to report outcomes in a large clinical cohort of patients using both IS and CLS fixation.

A retrospective review of radiographic, clinical, and patient-reported outcomes following ligament reconstruction from a Level 1 trauma centre was completed. Eligible patients were identified via electronic medical records using ICD-10 codes. Inclusion criteria were patients 18 years or older undergoing ACL, PCL, MCL, and/or LCL reconstruction between January 2018 and 2020 using IS and/or CLS fixation, with a minimum of six-month post-operative follow-up. Exclusion criteria were follow-up less than six months, incomplete radiographic imaging, and age less than 18 years. Knee dislocations (KD) were classified using the Schenck Classification. The primary outcome measure was implant removal rate. Secondary outcomes were revision surgery rate, deep infection rate, radiographic fixation failure rate, radiographic malposition, Lysholm and Tegner scores, clinical graft failure, and radiographic graft failure. Radiographic malposition was defined as implants over 5 mm off bone or intraosseous deployment of the suspensory fixation device. Clinical graft failure was defined as a grade II or greater Lachman, posterior drawer, varus opening at 20° of knee flexion, and/or valgus opening at 20° of knee flexion. Radiographic failure was defined when over 5 mm, 3.2 mm, and/or 2.7 mm of side-to-side difference occurred using PCL gravity stress views, valgus stress views, and/or varus stress views, respectively. Descriptive statistics were used.

Sixty-three consecutive patients (mean age = 41 years, range = 19-58) were included. A total of 266 CLS fixation with Ultrabuttons and 135 IS were used. Mean follow-up duration was 383 days. Most injuries were KD type II and III. Graft revision surgery rate was 1.5%. Intraosseous deployment occurred in 6.2% and 17% had implants secured in soft tissue, rather than on bone. However, the implant removal rate was only 6.2%. Radiographic PCL gravity stress views demonstrated an average of 1.2 mm of side-to-side difference with 6.2% meeting criteria for radiographic failure. A single patient met radiographic failure criteria for collateral grafts. Mean Lysholm and Tegner scores were 87.3 and 4.4, respectively, with follow-up beyond one year.

Both IS and CLS fixation demonstrate an extremely low revision surgery rate, a high rate of implant retention, excellent radiographic stability, and satisfactory patient-reported outcome scores.

Incorrect implant deployment was seen in a total of 17% of patients, yet none required implant removal. A single patient required graft revision due to implant failure.

Aim. The purpose of the study was to assess the outcomes of the surgical management of humeral shaft non-unions. Method. Between 2002 and 2010, 23 patients with humeral shaft non-unions underwent revision surgery. Nine were initially treated non-operatively, 11 with a plate, 1 with screws and 2 with an intramedullary nail. In previously operated patients with aseptic non-unions single stage revision was performed, while septic non-unions underwent a two stage revision. Revision stabilisation was by single or double plate fixation +/− external fixator, depending on bone loss and bone quality. Augmentation of fixation was with iliac crest bone graft in all cases. Data collected from the case notes and radiographs included: smoking habits, type and site of non-union, bone loss, infected or not, organism isolated, definitive stabilisation, augmentation used, post operative complications, further surgical interventions and time to radiological union. Results. There were 12 females and 11 males with an average age of 49 years (19–87 years). All but one were initially closed fractures. 12/23 smoked at least 15 cigarettes/day. Mean time from injury to revision surgery was 11 months. Sites of non-union were mid third (10), proximal third (8) and distal third (5). 10/14 in the previous surgery group were infected and underwent two stage revision. Average time to radiological union was 5 months in the uninfected group and 9 months in the infected group. Successful union was achieved in all cases in the uninfected group (100%) and all but two case in the infected group (91%). Four patients needed unanticipated further interventions to achieve union. At discharge all successfully treated patients had good shoulder and elbow function. Conclusion. Successful union of humeral shaft non-unions can be achieved with double plate fixation and augmentation with ICBG. This has reduced the need for supplementary external fixation

Aim:. Historically, anterior decompression followed by posterior fusion has been the surgical management of choice in spinal tuberculosis. Due to theatre time being at a premium, we have evolved to performing anterior only debridement, allograft strut reconstruction and instrumentation for tuberculosis in the adult thoracic spine. The aim of this study is to review the safety and the efficacy of this treatment. Methods:. Twenty-eight adult thoracic tuberculosis patients were identified where anterior only surgery had been performed. These were all in the mid-thoracic spine as circumferential surgery is still favoured in thoracolumbar disease. The surgery was performed by a single surgeon at a tertiary hospital. Following transthoracic aggressive debridement, allograft humeral shafts were cut to size and inserted under compression and the spines instrumented with the use of screw-rod constructs into the body above and below. A retrospective review of clinical notes and radiological studies was performed. Results:. Twenty-seven of the patients presented with altered neurology; 2 had only sensory changes while 25 presented with paraparesis; 22 of these patients were unable to walk. The average surgical time was 2 hours 20 minutes with a median blood loss of 726 ml. The majority of patients had 2 vertebral bodies involved and required an average of a 4 body fusion. Surgical complications included inadvertent opening of the diaphragm in 1 patient and 1 patient deteriorated neurologically post operatively. 21 of 28 patients recovered to independent mobility at their latest follow-up appointment. 1 patient showed no recovery, 3 had some motor recovery that was not useful, 1 had some sensory but no motor recovery. 16 of 28 patients have documented bony fusion with no evidence of instrumentation failure in any patients. Conclusion:. Anterior only debridement, allograft strut reconstruction and instrumented fusion for the treatment of thoracic spinal tuberculosis is a safe and effective alternative to circumferential surgery in the adult patient

In Asia, traditional bands are placed around limbs of children to ward off evil spirits and ensure good health. This practice may lead to the Rubber Band Syndrome (RBS) a rare paediatric condition described mainly in Asia which results from a forgotten thread or elastic band applied to the limb of a child. Because pressure necrosis beneath the band is slow, rapid healing of tissues over the band can obscure its presence. This makes the condition difficult to diagnose and distinguish from other infective conditions. This study presents the first case of RBS reported in South Africa.

Case: The patient aged 18 months presented with a swollen hand, circumferential scar and discharging sinus on the wrist. A radiograph was negative. In theatre the volar wound was explored. Debridement revealed a yellow rubber band deep to the wrist flexors and neurovascular bundles but superficial to the extensor retinaculum. Complete removal of the rubber band and antibiotics was followed by rehabilitation with Occupational Therapy. At a 3 month follow up, there was no evidence of infection, sensation was normal and motor function of the hand restored.

In this case, the structures through which the band had already passed were intact and overlay the band. It would appear that as the band slowly eroded the underlying structures it was followed by healing along the tract until the band exited deep to these structures. The band may come to rest on bone causing osteomyelitis if allowed to progress.

A high index of suspicion of RBS is needed in children presenting with a circumferential scar and a non-healing wound, especially on the wrist or ankle. Careful debridement and complete removal of the band are essential to resolve symptoms and restore function

Few studies have examined the socio-economic impact of complications requiring surgery following initial surgical management of proximal femoral fractures. Our hypothesis was that there would be a significant difference in the cost, mortality rate and ultimate discharge location in patients requiring further surgery after their index procedure for a proximal femoral fracture compared to a matched control group. This was a retrospective matched cohort study of all proximal femoral fractures presenting to the John Radcliffe Hospital over a five year period. Data had been collected prospectively in a standard manner. The total cost of treatment for each patient was calculated by separating the treatment costs into its components. Mortality data was retrieved from the Office of National Statistics and data were analysed using SPSS statistics software, with a p value of less than 0.05 considered significant. There were 2360 proximal femoral fractures in 2257 patients. Of this group, 144 (6.1%) required further surgical intervention due to a complication of the primary procedure. Mean age at time of fracture was 82.59 years; 81.6% women. Mean cost of treatment in those cases with complications was £18,731 compared to £8,575 for uncomplicated cases (p=0.00) with a mean length of stay of 62.8 days and 32.7 days respectively (p=0.00). Mortality probability of cases was significantly higher than the control group with a mean survival of 209 days versus 496 days for controls (p= 0.035) and patients with complications were statistically less likely to return to their own home (p< 0.01). The socio-economic impact of complications following treatment of proximal femoral fractures is important in this current economic climate. Greater awareness and understanding are warranted. Recognition of potential risk factors for complications may allow earlier detection of potential cases and thereby reduce their number and in turn the socioeconomic cost

Abstract

Background

With the increasingly accepted method of suprapatellar tibial nailing for tibial shaft fractures, we aimed to compare intraoperative and postoperative outcomes of infrapatellar (IP) vs suprapatellar (SP) tibial nails.

Methods

A retrospective cohort analysis of 58 patients. 34 SP tibial nails over 3 years versus 24 IP tibial nails over a similar time frame. We compared; radiation exposure, patient positioning time (PPT), non-union rate and follow-up time. Knee pain in the SP group was evaluated, utilising the Hospital for Special Surgery (HSS) Knee injury and Osteoarthritis outcome score (KOOS).


Aims

To compare the efficacy of decompression alone (DA) with i) decompression and fusion (DF) and ii) interspinous process device (IPD) in the treatment of lumbar stenosis with degenerative spondylolisthesis. Outcomes of interest were both patient-reported measures of postoperative pain and function, as well as the perioperative measures of blood loss, operation duration, hospital stay, and reoperation.

Methods

Data were obtained from electronic searches of five online databases. Included studies were limited to randomised-controlled trials (RCTs) which compared DA with DF or IPD using patient-reported outcomes such as the Oswestry Disability Index (ODI) and Zurich Claudication Questionnaire (ZCQ), or perioperative data.

Patient-reported data were reported as part of the systematic review, while meta-analyses were conducted for perioperative outcomes in MATLAB using the DerSimonian and Laird random-effects model. Forest plots were generated for visual interpretation, while heterogeneity was assessed using the I2-statistic.

This review compares the outcomes and complication rates of three surgical strategies used for the management of symptomatic os acromiale. The purpose of this study was to help guide best practice recommendations.

A systematic review of nine prospective studies, seven retrospective studies, and three case studies published across ten countries between 1993 and 2018 was performed. Adult patients (i.e., ≥ 18 years of age) with a symptomatic os acromiale that failed nonoperative management were included in this review. Surgical techniques utilized within the included studies include excision, acromioplasty, and open reduction and internal fixation (ORIF). The primary outcomes of interest included patient satisfaction and return to activity. Range of motion and several standardized outcome measurement tools were also included in the final analysis.

Patient satisfaction was highest in the excision and ORIF groups, with 92% and 82% of patients reporting good to excellent postoperative results, respectively, compared to 63% in the acromioplasty group. All three patient groups experienced improved postoperative objective scores (i.e., patient-reported outcome scores and active range of motion). The excision group experienced a complication rate of 1%, while the acromioplasty group experienced a complication rate of 11% and the ORIF group a rate of 67%.

This study reports on the largest sample of patients who have undergone surgical treatment for a symptomatic os acromiale. We have demonstrated that excision of the os with repair of the deltoid resulted in the best clinical outcomes with the least complications. In healthy adult patients with a large os fragment and a normal rotator cuff, surgical fixation may provide increased preservation of deltoid function while offering good patient satisfaction. Such patients should be aware that they are at increased risk of requiring a second procedure due to symptomatic hardware following ORIF.

Twenty-two patients with piriformis syndrome underwent surgery between October 1995 and February 2002. The mean age was 56 years (range 28-90). Only 2 patients (9%) gave a history of trauma to the ipsilateral buttock. All the patients complained of deep buttock pain, which radiated to the ankle in 14 (63%), the calf in 4 (18%) and the thigh in 3 patients (13.6%). The symptoms were chronic, with a mean of 70 months (range 12-192) and patients had been previously seen by a mean of 2.6 specialists (range 1-6). There was associated neurosensory loss in 11 patients (50%). The symptoms were exacerbated by passive stretching and active contraction of the piriformis muscle. MRI of the lumbar spine (every case) and gluteal region (8 cases) were negative, while NCV tests (20 cases) were positive in 11 patients (55%). Previous conservative measures such as physiotherapy (59%) and epidural (40%) had failed. Every patient had surgical division of the piriformis tendon at the greater trochanter by the senior author. Abnormal anatomy was identified in 6 cases (29%).

At 6 weeks (n=22), the symptoms had resolved in 6 patients (27%), were better in 8 (36%), no different in 7 (32%) and worse in 1 (4.5%). After a mean follow-up of 52 months (range 11-86), seven (35%) of patients were cured, 3 (15%) were better, 8 (40%) were no different, 2 (10%) were worse than pre-operatively and 2 had died of unrelated causes. Seventy-five percent of patients said that with hindsight they would undergo surgery again. One patient suffered a post-operative below knee DVT requiring no treatment.

Surgery for piriformis syndrome in this selected group of patients led to an improvement or resolution of chronic symptoms in 64% of patients at 6 weeks, and 50% of patients after a mean follow-up of 52 months with minimal associated morbidity.

Hip dysplasia represents a wide spectrum of disease, and interest in the treatment of the disorder has increased with the development of newer surgical techniques and a greater understanding of young adult hip disorders. National hospital episode statistics (HES) were studied from 1999 to 2010. This data remains the current best source of information on surgical procedures outside of dedicated registries. Age stratified data was analysed for 7 separately coded operations for the treatment of hip dysplasia. Overall in the paediatric population there were 898 procedures in 2010 compared to 793 procedures in 2000, but with no detectable trend across that period. Equally, there were no great fluctuations in the small numbers of arthroplasty procedures recorded in either the paediatric or adult populations. There was, however, a clear increase in surgery being performed in adult patients. 210 primary pelvic osteotomies were performed in 2010, compared with only 77 in 2000, with a noticeable increase from 2005 onwards. A similar trend in other extra-articular procedures is seen, rising from 2 to 55 per year over the period studied. Overall, the level of surgical intervention has steadily risen from 104 procedures in 2000 to 422 in 2010, representing a fourfold rise in the number of operations being performed for hip dysplasia in the adult population over an 11 year period. The reasons for this are unclear. It may reflect improvements in the ability to diagnose and intervene earlier to prevent disease progression, but further research is also needed to better define the aetiology underlying these cases that present to the hip surgeon later in life.

Introduction

Meniscal tears in middle-aged patients are common. There is a lack of consensus regarding the optimum management of these injuries. Although arthroscopic partial meniscectomy (APM) is a frequently performed surgical option, literature has failed to prove its effectiveness over conservative approaches.

Materials and Methods

We retrospectively reviewed 102 middle-aged (age > 40 years) patients who had been treated for meniscal tears between January 2010 and December 2012. We followed these patients up telephonically to assess knee outcome satisfaction rates, any early (6 weeks) improvements in pain and knee function and their pre-morbid and post-treatment activity levels (Tegner Activity Scale).

Microorganisms adhere to foreign bodies and form a biofilm, that protects against natural defence mechanisms and antibiotics. Because of these properties, surgery is required to cure an infected implant.

The type of surgery is different in early, delayed and late hematogenous infections.

Thorough debridement, with retention of the prosthesis but with exchange of the mobile parts, is possible in cases of early infections and in late hematogenous infections, if the implant is stable and the soft tissues intact.

If these conditions are not met and in delayed infections, the implant will have to be removed and a new prosthesis implanted.

This can be done in a one stage or in a two stage procedures; in most centres, one or the other procedure is favoured for all cases.

However, depending of the condition of the soft tissues, the presence or absence of concommittant diseases and the type of microorganism responsible for the infection, a decision toward either a one stage or a two stage procedure can be made.

Salvage procedures like a permanent resection, an arthrodesis or an amputation are less satisfactory for the patient.
