Aims. This study evaluates the association between consultant and hospital volume and the risk of re-revision and 90-day mortality following first-time revision of primary hip arthroplasty for aseptic loosening. Methods. We conducted a cohort study of first-time, single-stage revision hip arthroplasties (RHAs) performed for aseptic loosening and recorded in the National Joint Registry (NJR) data for England, Wales, Northern Ireland, and the Isle of Man between 2003 and 2019. Patient identifiers were used to link records to national mortality data, and to NJR data to identify subsequent re-revision procedures. Multivariable Cox proportional hazard models with restricted cubic splines were used to define associations between volume and outcome. Results. Among 12,961 RHAs there were 513 re-revisions within two years, and 95 deaths within 90 days of surgery. The risk of re-revision was highest for a consultant’s first RHA (hazard ratio (HR) 1.56 (95% CI 1.15 to 2.12)) and remained significantly elevated for their first 24 cases (HR 1.26 (95% CI 1.00 to 1.58)). Annual consultant volumes of five/year were associated with an almost 30% greater risk of re-revision (HR 1.28 (95% CI 1.00 to 1.64)) and 80% greater risk of 90-day mortality (HR 1.81 (95% CI 1.02 to 3.21)) compared to volumes of 20/year. RHAs performed at hospitals which had cumulatively undertaken fewer than 167 RHAs were at up to 70% greater risk of re-revision (HR 1.70 (95% CI 1.12 to 2.59)), and those having undertaken fewer than 307 RHAs were at up to three times greater risk of 90-day mortality (HR 3.05 (95% CI 1.19 to 7.82)). Conclusion. This study found a significantly higher risk of re-revision and early postoperative mortality following first-time single-stage RHA for aseptic loosening when performed by lower-volume consultants and at lower-volume institutions, supporting the move towards the centralization of such cases towards higher-volume units and surgeons. Aims. This study describes the variation in the annual volumes of revision hip arthroplasty (RHA) undertaken by consultant surgeons nationally, and the rate of accrual of RHA and corresponding primary hip arthroplasty (PHA) volume for new consultants entering practice. Methods. National Joint Registry (NJR) data for England, Wales, Northern Ireland, and the Isle of Man were received for 84,816 RHAs and 818,979 PHAs recorded between April 2011 and December 2019. RHA data comprised all revision procedures, including first-time revisions of PHA and any subsequent re-revisions recorded in public and private healthcare organizations. Annual procedure volumes undertaken by the responsible consultant surgeon in the 12 months prior to every index procedure were determined. We identified a cohort of ‘new’ HA consultants who commenced practice from 2012 and describe their rate of accrual of PHA and RHA experience. Results. The median annual consultant RHA volume, averaged across all cases, was 21 (interquartile range (IQR) 11 to 34; range 0 to 181). Of 1,695 consultants submitting RHA cases within the study period, the top 20% of surgeons by annual volume performed 74.2% of total RHA case volume. More than half of all consultants who had ever undertaken a RHA maintained an annual volume of just one or fewer RHA, however, collectively contributed less than 3% of the total RHA case volume. Consultant PHA and RHA volumes were positively correlated. Lower-volume surgeons were more likely to undertake RHA for urgent indications (such as infection) as a proportion of their practice, and to do so on weekends and public holidays. Conclusion. The majority of RHAs were undertaken by higher-volume surgeons. There was considerable variation in RHA volumes by indication, day of the week, and between consultants nationally. The rate of accrual of RHA experience by new consultants is low, and has important implications for establishing an experienced RHA consultant workforce. The variables involved in a robotic THA can exceed 52: many parameters as pelvic orientation with CT scan, templating, offset, and leg-length, acetabular reaming, femoral osteotomy, mapping the anatomy; predefining safe zones, robotic execution, femoral head size, thickness of PE etc. with several variables for each parameter, with a total number of variables exceeding 52. This familiar number is the number of cards in a standard deck. The number of possible combinations (factorial 52! = 10^67) to shuffle the cards (and may be to perform a THA) is greater than the number of atoms on earth! Thinking that artificial intelligence and robotics can solve these problems, some surgeons and implant manufacturers have turned to artificial intelligence and robotics. We asked two questions:1) can robot with artificial intelligence really process 52 variables that represent 10^67 combinations? 2) the safety of the technology was ascertained by interrogating Food and Drug Administration (FDA) database about software-related recalls in computer-assisted and robotic arthroplasty [1], between 2017 and 2022. 1). The best computers can only calculate around 100 thousand billion combinations (10^14), and with difficulty: it takes more than 100 days to arrive at this number of digits (10^14) after the decimal point for the number π (pi). We can, therefore, expect the robot to be imperfect. 2). For the FDA software-related recalls, 4634 units were involved. The FDA determined root causes were: software design (66.6%), design change (22.2%), manufacturing deployment (5.6%), design manufacturing process (5.6%). Among the manufacturers’ reasons for recalls, a specific error was declared in 88.9%. a coding error in 43.8%. 94.4% software-related recalls were classified as class 2. Return of the device was the main action taken by firms (44.4%), followed by software update (38.9%). 3). In the same period, no robot complained about its surgeon!. Hip surgeon is as intelligent as a robot and almost twice as safe

Aims. Total hip arthroplasties (THAs) are performed by surgeons at various stages in training with varying levels of supervision, but we do not know if this is safe practice with comparable outcomes to consultant-performed THA. Our aim was to examine the association between surgeon grade, the senior supervision of trainees, and the risk of revision following THA. Methods. We performed an observational study using National Joint Registry (NJR) data. We included adult patients who underwent primary THA for osteoarthritis, recorded in the NJR between 2003 and 2016. Exposures were operating surgeon grade (consultant or trainee) and whether or not trainees were directly supervised by a scrubbed consultant. Outcomes were all-cause revision and the indication for revision up to ten years. We used methods of survival analysis, adjusted for patient, operation, and healthcare setting factors. Results. We included 603,474 THAs, of which 58,137 (9.6%) procedures were performed by a trainee. There was no association between surgeon grade and all-cause revision up to ten years (crude hazard ratio (HR) 1.00 (95% confidence interval (CI) 0.94 to 1.07); p = 0.966), a finding which persisted with adjusted analysis. Fully adjusted analysis demonstrated an association between trainees operating without scrubbed consultant supervision and an increased risk of all-cause revision (HR 1.10 (95% CI 1.00 to 1.21); p = 0.045). There was an association between trainee-performed THA and revision for instability (HR 1.14 (95% CI 1.01 to 1.30); p = 0.039). However, this was not observed in adjusted models, or when trainees were supervised by a scrubbed consultant. Conclusion. Within the current training system in England and Wales, appropriately supervised trainees achieve comparable THA survival to consultants. Trainees who are supervised by a scrubbed consultant achieve superior outcomes compared to trainees who are not supervised by a scrubbed consultant, particularly in terms of revision for instability. Aims. Total hip arthroplasty (THA) using the direct anterior approach (DAA) is undertaken with the patient in the supine position, creating an opportunity to replace both hips under one anaesthetic. Few studies have reported simultaneous bilateral DAA-THA. The aim of this study was to characterize a cohort of patients selected for this technique by a single, high-volume arthroplasty surgeon and to investigate their early postoperative clinical outcomes. Methods. Using an institutional database, we reviewed 643 patients who underwent bilateral DAA-THA by a single surgeon between 1 January 2010 and 31 December 2018. The demographic characteristics of the 256 patients (39.8%) who underwent simultaneous bilateral DAA-THA were compared with the 387 patients (60.2%) who underwent staged THA during the same period of time. We then reviewed the length of stay, rate of discharge home, 90-day complications, and readmissions for the simultaneous bilateral group. Results. Patients undergoing simultaneous bilateral DAA-THA had a 3.5% transfusion rate, a 1.8 day mean length of stay, a 98.1% rate of discharge home, and low rates of 90-day infection (0.39%), dislocation (0.39%), periprosthetic fracture (0.77%), venous thromboembolism (0%), haematoma (0.39%), further surgery (0.77%), and readmission (0.77%). These patients were significantly younger (mean 58.2 years vs 62.5 years; p < 0.001), more likely to be male (60.3% vs 46.5%; p < 0.001), and with a trend towards having a lower mean BMI (27.8 kg/m. 2. vs 28.4 kg/m. 2. ; p = 0.071) than patients who underwent staged bilateral DAA-THA. Conclusion. Patients selected for simultaneous bilateral DAA-THA in a single surgeon’s practice had a 3% rate of postoperative transfusion and a low rate of complications, readmissions, and discharge to a rehabilitation facility. Simultaneous bilateral DAA-THA appears to be a reasonable and safe form of treatment for patients with bilateral symptomatic osteoarthritis of the hip when undertaken by an experienced arthroplasty surgeon with appropriate selection criteria. Annual consultant volumes of five/year were associated with an almost 30% greater risk of re-revision (HR 1·28 (95%CI 1·00 to 1·64)) and 80% greater risk of 90-day mortality (HR 1·81 (95%CI 1·02 to 3·21)) compared to volumes of 20/year. RHR performed at hospitals which had cumulatively undertaken fewer than 168 RHR were at up to 70% greater risk of re-revision (HR 1·70 (95% CI 1·12 to 2·60)), and those having undertaken fewer than 309 RHR were at up to three times greater risk of 90-day mortality (HR 3·06 (95% CI 1·19 to 7·86)). Aims. To establish whether there was a consensus among the members of the Hip Society (HS) on the role of direct anterior approach (DAA) contemporary primary total hip arthroplasty (THA). Methods. An online survey was sent to all 112 active and senior members of the HS, to which 71 members responded. The survey was constructed to determine whether they believed that evidence-based medicine proves, in modern clinical practice, that the DAA has significant benefits compared to risks when contrasted with other approaches. In addition, they were asked if they currently used the DAA. Results. While only 16.9% (12/71) of respondents had been trained in a generic anterior approach during residency, 49.3% (35/71) had used the DAA in their clinical practice in the past or were using it at the present time. Unexpectedly, 42.9% (15/35) of respondents who had used the DAA in the past had abandoned it by the time of this survey. Only 22.5% (16/71) of all respondents believed that evidence-based medicine proves that the DAA has significant benefits compared to risks in contrast to other approaches. Conclusion. A comprehensive literature review found only three prospective randomized clinical trials (RCT) comparing the DAA with another approach with greater than one-year follow-up. Two showed minor benefits within the early postoperative period only, and one of those showed poorer mid-term results. Most of the published comparison studies with short follow-up show longer surgical times and greater blood loss for the DAA, and many three-month comparison studies show higher complication rates for the DAA using a proprietary traction table. The complications included problems with wound healing, lateral femoral cutaneous nerve injury, femoral component loosening, and femoral fractures. Because of the lack of evidence from RCTs showing superiority of the DAA over other approaches and reports of higher complications, the opinion of a large majority (77.5%; 55/71) of HS surgeons was that the DAA lacks sufficient evidence to warrant its use. Aims. The aim of this study was to conduct evidence synthesis on the available published literature of the impact of the training status of the operating surgeon (trainee vs. consultant) on the survival and revision rate of primary hip and knee replacements. Patients and Methods. We conducted a systematic review according to Cochrane guidelines. Separate searches were performed for hip and knee replacements, with meta-analysis and presentation of results in parallel. We searched MEDLINE and Embase databases from inception to 17 September 2019 and included controlled trials and cohort studies reporting implant survival estimates, or revision rates of hip and knee replacements according to the grade of the operating surgeon. This study was registered with PROSPERO (CRD42019150494). Results. 8 studies (5 hip papers and 3 knee papers) met the inclusion criteria. There was no significant difference in the survival estimates for total hip replacements (THRs) performed by trainees compared to consultants at 5-years follow-up (97.9% vs 98.1%, p = 0.74). Furthermore, there was no significant difference in the revision rate of THRs performed by trainees and consultants at both 5 and 10-year intervals of follow-up (relative risk [RR]: 5yrs = 0.88 (95% CI: 0.46, 1.70; P = 0.71); 10yrs = 0.68 (95% CI: 0.37, 1.26; P = 0.22)). There was no significant difference in the survival estimates at 10-years for total knee replacements (TKRs) performed by trainees compared to consultants (96.2% vs 95.1%, p=0.49). Conclusion. There is no evidence in the existing literature that trainee surgeons have worse outcomes than their consultant colleagues, in terms of the survival, or rate of revision of hip and knee replacements at 5–10 years follow-up. This may mean that there is genuinely no difference or that, in the context of contemporary training programmes, appropriate case-mix selection and supervision of trainees is currently employed

Total hip replacements (THRs) are performed by surgeons at various stages in their training, with varying levels of senior supervision. There is a balance between protecting training opportunities for the next generation of surgeons, while limiting the exposure of patients to unnecessary risk during the training process. The aim of this study was to examine the association between surgeon grade, the senior supervision of trainees, and the risk of revision following THR. We included 603 474 primary THRs recorded in the National Joint Registry for England, Wales, Northern Ireland, and the Isle of Man (NJR) between 2003 and 2016 for an indication of osteoarthritis. Exposures were the grade of the surgeon (consultant or trainee), and whether trainees were supervised by a scrubbed consultant or not. Outcomes were all-cause revision, the indication for revision, and the temporal variation in risk of revision (all up to 10 years). Net failure was calculated using Kaplan-Meier analysis and adjusted analyses used Cox regression and flexible parametric survival analysis (adjusted for patient, operative, and unit level factors). There was no association between surgeon grade and all-cause revision up to 10 years (crude hazard ratio (HR) 0·999, 95% confidence interval (CI) 0.936–1.065; p=0.966); a finding which persisted with adjusted analysis. Adjusted analysis demonstrated an association between trainees operating without supervision by a scrubbed consultant and an increase in all-cause revision (HR 1.100, 95% CI 1.002–1.207; p=0.045). There was an association between the trainee-performed THRs and revision due to instability (crude HR 1.143, 95% CI, 1.007–1.298; p=0.039). However, this was not observed in fully adjusted models, or when trainees were supervised by a scrubbed consultant. Within the current training system in the United Kingdom, trainees achieve comparable outcomes to consultant surgeons when supervised by a scrubbed consultant. Revision rates are higher when trainees are not supervised by a scrubbed consultant but remain within internationally recognised acceptable limits

Background. For total hip arthroplasty (THA), cognitive training prior to performing real surgery may be an effective adjunct alongside simulation to shorten the learning curve. This study sought to create a cognitive training tool to perform direct anterior approach THA, validated by expert surgeons; and test its use as a training tool compared to conventional material. Methods. We employed a modified Delphi method with four expert surgeons from three international centres of excellence. Surgeons were independently observed performing THA before undergoing semi-structured cognitive task analysis (CTA) before completing successive rounds of electronic surveys until consensus. The agreed CTA was incorporated into a mobile and web-based platform. Forty surgical trainees (CT1-ST4) were randomised to CTA-training or a digital op-tech with surgical videos, before performing a simulated DAA THA in a validated fully-immersive virtual reality simulator. Results. Experts reached 100% consensus after five rounds. They defined THA in 46 steps and 52 decision points in 8 distinct procedural phases. Each phase comprised of a set of actions, cognitive demands, and critical errors and strategies. This CTA was mapped onto an open-access web-based learning tool [1]. Surgeons who prepared with CTA performed a simulated THA more efficiently (Time: 26 vs. 36 minutes and Procedural steps: 64 vs. 78), with fewer errors in instrument selection (22 vs 34 instances) and help required (6 vs. 19 instances), and with more accuracy (acetabular cup inclination error: 7° vs. 12°, anteversion error: 11° vs 19°) than those who prepared with conventional material. Discussion. This is the first validated CTA tool for arthroplasty. It provides structure for competency-based learning of this complex procedure. It is more effective at preparing orthopaedic trainees for a new procedure than conventional materials, for learning sequence, instrumentation and motor skills. Implications. Cognitive training combines education on decision making, knowledge and technical skill. It is a validated educational tool to upskill surgeons to perform hip arthroplasty and could replace current training and preparation methods for junior surgeons

Aims. The aim of this study was to develop a novel computational model for estimating head/stem taper mechanics during different simulated assembly conditions. Methods. Finite element models of generic cobalt-chromium (CoCr) heads on a titanium stem taper were developed and driven using dynamic assembly loads collected from clinicians. To verify contact mechanics at the taper interface, comparisons of deformed microgroove characteristics (height and width of microgrooves) were made between model estimates with those measured from five retrieved implants. Additionally, these models were used to assess the role of assembly technique—one-hit versus three-hits—on the taper interlock mechanical behaviour. Results. The model compared well to deformed microgrooves from the retrieved implants, predicting changes in microgroove height (mean 1.1 μm (0.2 to 1.3)) and width (mean 7.5 μm (1.0 to 18.5)) within the range of measured changes in height (mean 1.4 μm (0.4 to 2.3); p = 0.109) and width (mean 12.0 μm (1.5 to 25.4); p = 0.470). Consistent with benchtop studies, our model found that increasing assembly load magnitude led to increased taper engagement, contact pressure, and permanent deformation of the stem taper microgrooves. Interestingly, our model found assemblies using three hits at low loads (4 kN) led to decreased taper engagement, contact pressures and microgroove deformations throughout the stem taper compared with tapers assembled with one hit at the same magnitude. Conclusion. These findings suggest additional assembly hits at low loads lead to inferior taper interlock strength compared with one firm hit, which may be influenced by loading rate or material strain hardening. These unique models can estimate microgroove deformations representative of real contact mechanics seen on retrievals, which will enable us to better understand how both surgeon assembly techniques and implant design affect taper interlock strength. Introduction. We aim to evaluate the outcome of debridement and implant retention (DAIR) procedures performed for primary total hip prosthetic joint infections (PJI) and to identify factors correlating with a successful outcome. Methods. Patients were identified from theatre records. Electronic and paper notes were reviewed. Results. Fifty-four DAIR procedures for infected, elective, primary total hip replacements were performed between 2010 and 2017. Complete records were available for 44 procedures. There were 22 males and 22 females. Mean age was 71 (38–89) years. Mean follow-up was 21.6 (2–52) months. Ninety-one percent of DAIRs were performed for exogenous infections. Procedures were performed on average 23 days from the primary procedure in exogenous infections and 11 days from onset of symptoms in haematogenous infections. Nine of 40 cases for exogenous infection were performed more than 28 days from the primary procedure. The procedure resulted in a successful outcome in 34 cases (77%). There was no significant difference in the time to DAIR from the primary procedure comparing successful and unsuccessful cases. A successful outcome was associated with changing the femoral head, the procedure being performed by a revision hip surgeon, not inserting gentamicin impregnated fleece, and positive identification of the infecting organism. Discussion. Prompt treatment with DAIR of suspected primary hip PJI can result in a high rate of successful outcome. The femoral head should always be exchanged and a delay to DAIR is preferable to the procedure being performed by a surgeon who does not routinely perform revision hip surgery

Aims. The long-term functional outcome of total hip arthroplasty (THA) performed by trainees is not known. A multicentre retrospective study of 879 THAs was undertaken to investigate any differences in outcome between those performed by trainee surgeons and consultants. . Patients and Methods. A total of 879 patients with a mean age of 69.5 years (37 to 94) were included in the study; 584 THAs (66.4%) were undertaken by consultants, 138 (15.7%) by junior trainees and 148 (16.8%) by senior trainees. Patients were scored using the Harris Hip Score (HHS) pre-operatively and at one, three, five, seven and ten years post-operatively. Surgical outcome, complications and survival were compared between groups. The effect of supervision was determined by comparing supervised and unsupervised trainees. A primary univariate analysis was used to select variables for inclusion in multivariate analysis. . Results. There was no evidence that the grade of the surgeon had a significant effect on the survival of the patients or the rate of revision (p = 0.987 and 0.405, respectively) up to 12 years post-operatively. There was no significant difference in post-operative functional HHS or total HHS among consultants, junior and seniors up to ten years post-operatively (p = 0.401 and 0.331), respectively. There was no significant difference in hospital stay (p = 0.855) between different grades of surgeons. There was no evidence that the level of supervision had an effect on the survival of the patients or the rate of revision (p = 0.837 and 0.203, respectively) up to 12 years post-operatively. There was no significant difference between supervised and unsupervised trainee groups in post-operative functional HHS or total HHS up to ten years post-operatively (p = 0.213 and 0.322, respectively). There was no significant difference in the mean hospital stay between supervised and unsupervised trainees (p = 0.908). . Take home message: This study suggests that when trainees are appropriately supervised, they can obtain results comparable with those of their consultant colleagues when performing THA. Introduction. Custom flanged acetabular components (CFAC) have been shown to be effective in treating complex acetabular reconstructions in revision total hip arthroplasty (THA). However, the specific patient factors and CFAC design characteristics that affect the overall survivorship remain unclear. Once the surgeon opts to follow this treatment pathway, numerous decisions need to be made during the pre-operative design phase and during implantation, which may influence the ultimate success of CFAC. The goal of this study was to retrospectively review the entire cohort of CFAC cases performed at a large volume institution and to identify any patient, surgeon, or design factors that may be related to the long-term survival of these prostheses. Methods. We reviewed 96 CFAC cases performed in 91 patients between 2004 and 2017, from which 36 variables were collected spanning patient demographics, pre-operative clinical and radiographic features, intraoperative information, and implant design characteristics. Patient demographics and relevant clinical features were collected from individual medical records. Radiographic review included analysis of pre-operative radiographs, computer tomographic (CT) scans, and serial post-operative radiographs. Radiographic failure was defined as loosening or gross migration as determined by a board-certified orthopedic surgeon. CFAC implant design characteristics and intra-operative features were collected from the design record, surgical record and post-operative radiograph for each case respectively. Two sets of statistical analyses were performed with this dataset. First, univariate analyses were performed for each variable, comprising of a Pearson chi-square test for categorical variables and an independent t-test for continuous variables. Second, a random forest supervised machine learning method was applied to identify the most influential variables within the dataset, which were then used to perform a bivariable logistic regression to generate odds ratios. Statistical significance for this study was set at p < 0.05. Results. Radiographic failures occurred in 22/96 (23%) of cases with 12/96 (13%) undergoing re-revision (time to revision: Mean 25.1 months; Range: 3 – 84, SD 26.5). No relationship between radiographic failure and the preoperative Paprosky grade or the presence of a discontinuity was observed. The rate of radiographic failure (loosening and/or migration) was inversely associated with age, with increased failure seen in patients who were younger at the time of surgery; (mean age: 54.4±13.0 v. 64.8±11.4 years; p=0.007). Patients whose initial diagnosis was not osteoarthritis were more likely to fail than those with primary OA (OR: 3.79, p=0.0173) and were younger at the time of surgery (p=0.013). The presence of ischial screws from previous surgery (28%) was also an independent risk factor for failure (OR: 3.11, p=0.021). Random forest analysis identified the age at index procedure and the location of the inferior-most ischial screw as the most sensitive predictors of radiographic failure. As patient age at the time of surgery increased, there was subsequent a decreased rate of failure (OR: 0.93 odds ratio per year, p =0.005). When the bottom-most ischial screw was within the top half of the obturator foramen, it was 4 times more likely to fail than when this screw was located at the bottom half of the obturator foramen (OR = 3.98, p=0.046) (p < 0.05). Discussion and Conclusion. This study was able to identify the patient and design variables predictive of survival of CFAC prostheses used in complex revision THA. Younger patients (<55years) are at increased risk for failure either due to a more active lifestyle or because they have a non-OA primary diagnosis that predisposes them to earlier THA. Compromised ischial bone stock or inadequate ischial fixation both had a significant impact on CFAC implant survivorship as both the presence of pre-CFAC ischial screw fixation and lack of inferior ischial fixation correlated with increased rate of failure. These findings highlight the importance of rigid ischial fixation sufficient to resist the high pull-out forces generated during activities of daily living

Introduction. Femoroacetabular impingement(FAI) is a relatively common cause causes of hip pain and dysfunction in active young adults. The concept of FAI was popularized by Ganz et al in early 2000s. Surgical treatment for FAI has been widely employed over the last two decades. The long term outcome of femoroacetabular osteoplasty (FAO) and risk factors for long-term failure of FAO is less studied. The goal of this single surgeon series is to identify the long term outcome of FAO (minimum 10-year follow-up) and risk factors for surgical treatment failure in these patients. Methods. The prospective database on hip joint preservation identified 1,120 patients who have undergone FAO between January 2005-June 2019. Of these 164 patients (178 hips) have a minimum 10-year follow-up (range, 10–14). The database collects detailed information on patient demographics, clinical history, radiographic and cross sectional imaging findings, intraoperative findings (site and size of chondral lesion, labral tear, subchondral cyst, size of cam lesion, etc), and patient outcome using the modified HHS, SF-36, and UCLA activity score. Patients are contacted on an annual basis and the functional outcome instruments administered. Of 1,120 patients, 122 patients have been lost to follow-up. Results. The mean age of patients in the cohort is 34.3±10.4 years and 39.6% of patients are female. FAO resulted in an improvement in mean mHHS (58.2±3.9 to 86.4±3.2) and SF-36(60.4±4 to 85±4.1) in 89% of patients. At the latest follow up 8.1% of patients have undergone THA. The study found that older age, longer preoperative symptomatic period, higher preoperative alpha angle, presence of hip dysplasia and acetabular retroversion, inability to repair acetabular labrum, and full thickness acetabular chondral lesion were associated with higher risk of failure of treatment. Conclusion. Patients with symptomatic FAI who undergo surgery experience pain relief and functional improvement that appears to endure over a decade in the majority. This study on a large cohort with long term follow-up has also identified patients who are at higher risk of failure. The study has also shed light on some interesting findings such as labral tear being very common in patients without hip symptoms and others that will be presented. Introduction. The utility and yield of the current practice of routine screening of asymptomatic patients after primary total hip arthroplasty (THA) and total knee arthroplasty (TKA) is unclear. The purpose of this prospective survey study was to determine the utility of the routine on year follow up visit primary THA and TKA. Methods. We prospectively enrolled all patients undergoing primary THA and TKA. At one-year follow-up, patients were asked to complete a survey that asked about satisfaction with the and if they thought the visit was worthwhile. Surgeons also completed a survey which asked if any intervention was done, if any problems were diagnosed/avoided, and if the visit was worthwhile. Data was analyzed and compared between patients and surgeons, and was also compared to the need for any additional interventions. Results. Between October 2017 and June 2018, 512 patients underwent primary THA or TKA. Of these, passive one-year follow-up was obtained in 195 (38%). The final cohort consisted of 195 patients; 102 THAs and 94 TKAs with mean follow-up of 378 days (range, 276–480 days). Patients reported a mean 4.64 rating when asked if the visit was worthwhile (5-point scale, 1=not worthwhile, 5=very worthwhile). No interventions were performed or ordered at 56.3% of visits. Physicians reported that complications were avoided because of the visit at 23.03% of visits and 66.8% of visits were worthwhile (yes/no). 49.44% of visits where no intervention was ordered were rated as worthwhile, whereas 90.3% of visits with interventions ordered were rated as worthwhile. Conclusions. Patients generally thought that their follow up visit was worthwhile, even when no intervention was ordered or issue was addressed. In the visits without any interventions, over half of physicians thought the visit was not worthwhile. We compared patient satisfaction with surgeon satisfaction after total hip arthroplasty (THA) in a group of 193 patients (200 THAs, mean follow-up six years) using a visual analogue scale (VAS), and two objective and two subjective scoring systems. We also determined the survival rate with different endpoints. For the 121 hips available for clinical follow-up, we did not find a significant difference in satisfaction between patient and surgeon. In a subgroup with low patient satisfaction, the surgeon was more satisfied than the patient (p = 0.04). The correlation between the patient satisfaction VAS and the different subjective and objective scoring systems suggests that pain during activity is the most important factor for the patient. The survivorship at six years decreased from 96.6% to 83.7% if dissatisfaction (VAS > 20) was added to revision as an endpoint in the survival analysis. The patient satisfaction VAS provides additional information to evaluate the outcome of THA. We recommend the use of both subjective and objective scoring systems to evaluate the outcome of THA

Between December 2004 and June 2006, 136 patients (156 total hip replacements), were sent from the waiting list of the Cardiff Vale NHS Trust to the NHS Treatment Centre, Weston-super-Mare, in an attempt to reduce the waiting time for total hip replacement. Because of concerns about their outcome, each patient was contacted and invited to attend a review appointment with a consultant specialising in hip and revision hip replacement.

A total of 98 patients (113 hips) were reviewed after a mean of 23 months (11 to 30). There were 104 cemented hips, seven hybrid and two cementless. An acetabular inclination of > 55° was seen in 18 (16%). Radiolucent lines around the acetabular component were seen in 76 (67%). The femoral component was in more than 4° of varus in 47 (42%). The medial floor had been breached in 13 (12%) and there was a leg-length discrepancy of more than 1 cm in ten (9%). There were three dislocations, one femoral fracture, one pulmonary embolus, one deep infection and two superficial wound infections.

To date, 13% (15 hips) have been revised and a further 4% (five hips) await revision, mostly for a painful loose acetabular component. The revision rate far exceeds the 0.5% five-year failure rate reported in the Swedish Registry for the components used. This initiative and the consequent need for correction of the problems created, has significantly increased the workload of our unit.

At the end of 2018, the NZ Joint Registry introduced a “Surgeon Outlier” policy, whereby each year, if an individual surgeons’ lower 95% confidence interval of their revision rate, measured in revision/100 component years(r/ocys), was above the NZ mean (0.71 r/ocys), that surgeon was required to audit their results with a nominated peer. This study investigates whether outlier surgeons also have high early (1 month and 1 year) revision rates. In 2018, 236 surgeons performed 9,186 total hip arthroplasties in NZ. At the end of 2018, 11 surgeons received notification they were outliers. Results from all surgeons for years 2016, 2017 and 2018 were combined to form the first (pre-notification) time interval, and results from years 2019, 2020 and 2021 were combined to form the second time interval (post-notification). Outlier surgeons performed 2001 total hip replacements in the first time interval and 1947 hips in the second. Early revision rates (1 month and 1 year) of both outlier and nonoutlier surgeons for both time intervals were analysed. Non-outlier surgeons had a consistent mean early revision rate of 0.75% at one month and 1.6% at one year for both time intervals. The 11 outlier surgeons had a higher earlier revision rate of 1.35% at one month and 2.45% at one year for the pre-notification time interval. These values reduced for the post-notification time interval to a revision rate of 1.23% for one month and 2.36% for one year. Poor joint registry results of individual surgeons are often attributed to a poor choice of prosthesis. This study shows early revision rates of outlier surgeons, where prosthesis selection has minimal influence, are also high. A slight improvement in early revision rates of outlier surgeons since introduction of the policy shows it is working

Patients with a high comorbidity burden (high-risk) can achieve similar improvements in quality of life compared to low-risk patients, but greater morbidity may deter surgeons from operating on these patients. Whether surgeon volume influences THA outcomes in high-risk patients has not been investigated. This study aimed to compare complication rates and implant survivorship in high-risk patients operated on by high volume (HV) and non-HV THA surgeons. Patients with Charlson Comorbidity Index ≥ 5 and American Society of Anesthesiologist Classification of 3 or 4 undergoing primary, elective THA between 2013 and 2021 were retrospectively reviewed. Patients were separated into groups based on whether they were operated on by a HV surgeon (defined as the top 25% of surgeons at our institution by number of primary THAs per year) or a non-HV surgeon. Groups were propensity matched 1:1 to control for demographic variables. A total of 1,134 patients were included in the matched analysis. Ninety day readmissions and revisions were compared between groups, and Kaplan-Meier analysis was used to evaluate implant survivorship within the follow-up period. Years of experience were comparable between Non-HV and HV surgeons (p=0.733). The HV group had significantly shorter surgical times (p<0.001), and shorter length of stay (p=0.009) compared to the Non-HV group. The HV group also had significantly fewer 90-day readmissions (p=0.030), all-cause revisions (p=0.023) and septic revisions (p=0.020) compared to the non-HV group at latest follow-up. The HV group had significantly greater freedom from all-cause (p=0.023) and septic revision (p=0.020) compared to the non-HV group. High-risk THA patients have fewer 90-day readmissions, all-cause revisions, septic revisions, as well as shorter length of stay when treated by HV surgeons. THA candidates with a high comorbidity burden may benefit from referral to high-volume surgeons to reduce procedural risk and improve postoperative outcomes
