Aims. The aims of this study were to determine if an increasing serum cobalt (Co) and/or chromium (Cr) concentration is correlated with a decreasing Harris Hip Score (HHS) and Hip disability and Osteoarthritis Outcome Score (HOOS) in patients who received the Articular Surface Replacement (ASR) hip resurfacing arthroplasty (HRA), and to evaluate the ten-year revision rate and show if sex, inclination angle, and Co level influenced the revision rate. Methods. A total of 62 patients with an ASR-HRA were included and monitored yearly postoperatively. At follow-up, serum Co and Cr levels were measured and the HHS and the HOOS were scored. In addition, preoperative patient and implant variables and the need for revision surgery were recorded. We used a linear mixed model to relate the serum Co and Cr levels to different patient-reported outcome measures (PROMs). For the survival analyses we used the Kaplan-Meier and Cox regression model. Results. We found that an increase of one part per billion (ppb) in serum Co and Cr levels correlated significantly with worsening of the HHS in the following year. This significant correlation was also true for the HOOS-Pain and HOOS-quality of life sub scores. The overall ten-year survival rate in our cohort was 65% (95% confidence interval (CI) 52.5 to 77.6). Cox regression analysis showed a significant hazard ratio (HR) of 1.08 (95% CI 1.01 to 1.15; p = 0.028) for serum Co level. No significance was found with sex or inclination angle. Conclusion. This study shows that increasing serum Co and Cr levels measured in patients with an ASR-HRA are predictive for deterioration in HHS and HOOS subscales in the following year. Increasing serum Co and Cr should forewarn both surgeon and patient that there is a heightened risk of failure. Continued and regular review of patients with an ASR-HRA implant by measurement of serum Co/Cr levels and PROMs remains essential. Cite this article: Bone Joint J 2023;105-B(7):775–782

Introduction. Modular dual-mobility constructs reduce the risk of dislocation after total hip arthroplasty (THA). However, questions about metal ions from the cobalt-chrome (CoCr) liner persist, and are particularly germane to patients being revised for adverse local tissue reactions (ALTR) to metal. We determined the mid-term serum Co and Cr levels after modular dual-mobilities used in revision and complex primary THAs, and specifically included patients revised for ALTR. Methods. Serum Co and Cr levels were measured prospectively in 22 patients with a modular dual-mobility construct and a ceramic femoral head. Patients with CoCr heads or contralateral THAs with CoCr heads were excluded. Mean age 64 years with 50% female. The mean follow-up was 4 years. Indications for modular dual-mobility were: periprosthetic joint infection treated with 2-stage exchange and subsequent reimplantation (n=8), ALTR revision (n=7), complex primary THA (n=6), and periprosthetic femoral fracture (n=1). Mean preoperative Co and Cr in patients revised for an ALTR were 29.7 µg/L and 21.5 µg/L, respectively. Results. Mean Co and Cr levels were 0.26 µg/L and 0.82 µg/L, respectively, at most recent follow-up. No patients had a cobalt level ≥1 µg/L. Only one patient had a chromium level ≥1 µg/L. That patient's chromium level was 12 µg/L at 57 months after revision THA for ALTR (and decreased 10-fold from a preoperative Cr of 113 µg/L). Conclusion. At a mean of 4 years, no patient with a modular dual-mobility construct and ceramic femoral head had elevated cobalt levels, including seven patients revised specifically for ALTR. While further studies are required, we support selective use of a modular dual-mobility construct in revision and complex primary THAs for patients at high risk for instability. Summary. At a mean of 4 years after revision or complex primary THAs with a modular dual-mobility construct and a ceramic head, none of 22 patients had serum cobalt levels that measured ≥1 µg/L

Aims

However, questions about metal ions from the cobalt-chromium (CoCr) liner persist, and are particularly germane to patients being revised for adverse local tissue reactions (ALTR) to metal. We determined the early- to mid-term serum Co and Cr levels after modular dual-mobility components were used in revision and complex primary THAs, and specifically included patients revised for ALTR.

Patients and Methods

Serum Co and Cr levels were measured prospectively in 24 patients with a modular dual-mobility construct and a ceramic femoral head. Patients with CoCr heads or contralateral THAs with CoCr heads were excluded. The mean age was 63 years (35 to 83), with 13 patients (54%) being female. The mean follow-up was four years (2 to 7). Indications for modular dual-mobility were prosthetic joint infection treated with two-stage exchange and subsequent reimplantation (n = 8), ALTR revision (n = 7), complex primary THA (n = 7), recurrent instability (n = 1), and periprosthetic femoral fracture (n = 1). The mean preoperative Co and Cr in patients revised for an ALTR were 29.7 μg/l (2 to 146) and 21.5 μg/l (1 to 113), respectively.

The purpose of this randomized controlled trial was to evaluate serum metal ion levels in patients undergoing THA with either a standard or modular dual-mobility bearing. Patients undergoing primary THA for osteoarthritis were randomized to receive either a modular dual-mobility or a standard polyethylene bearing. All patients received the same titanium acetabular and femoral component and a ceramic femoral head. Only patients without a prior history of metal implants in their body were eligible for inclusion, thus isolating serum metal ions to the prosthesis itself. Serum metal ion levels were drawn pre-operatively and at 1 year postoperatively. Power analysis determined that 40 patients (20 in each group) were needed to identify a clinically relevant difference in serum cobalt of 0.35 ng/ml (ppb) at 90% power assuming a pooled standard deviation of 0.31 ppb and alpha=0.05; an additional 30% were enrolled to account for potential dropouts. 53 patients were enrolled, with 22 patients in the modular dual-mobility group and 20 in the standard cohort with data available at one-year. No differences in the serum cobalt (0.17 ppb [range 0.07 to 0.50] vs. 0.19 ppb [range 0.07 to 0.62], p = 0.51) or chromium levels (0.19 ppb [range 0.05 to 0.56] vs. 0.16 ppb [range 0.05 to 0.61], p = 0.23) were identified. At 1 year postoperatively, no differences in serum cobalt or chromium levels were identified with this design of a modular dual mobility bearing when compared to a standard polyethylene bearing

The purpose of this randomized controlled trial was to evaluate serum metal levels in patients undergoing total hip arthroplasty (THA) with a conventional compared to a modular dual-mobility bearing. Patients undergoing primary THA for osteoarthritis were randomized to receive either a modular dual-mobility or conventional polyethylene bearing. All patients received the same titanium acetabular and femoral component and a ceramic femoral head. Serum metal levels were drawn pre-operatively then annually for a minimum of two years postoperatively. An a priori power analysis determined that 40 patients (20 per cohort) were needed to identify a clinically relevant difference in serum cobalt of 0.35 ng/ml (ppb) at 90% power. Forty-six patients were randomized to a modular dual-mobility (n=25) or conventional bearing (n=21) with 40 at a minimum follow-up of two years. No differences in serum cobalt (mean 0.14 ppb [range, 0.075–0.29] vs. 0.20 ppb [range, 0.075–0.57], p=0.39) or chromium levels (mean 0.14 ppb [range, 0.05–0.50] vs. 0.12 ppb [range, 0.05–0.35], p=0.65) were identified between the modular dual-mobility and conventional cohorts, respectively. There was no statistically significant difference in serum Co or Cr at two years postoperatively in subjects implanted with a ceramic head and this particular dual mobility bearing in comparison to a ceramic head and a conventional acetabular component. While modest expected elevations in serum Co and Cr were observed in the dual mobility group, in no case did the Co level exceed the laboratory reference range nor the threshold of one part per billion that has been associated with adverse local tissue reactions to mechanically-assisted crevice corrosion

Ceramic-on-ceramic (CoC) articulations in total hip arthroplasty (THA) have low wear, but the unique risk of fracture. After revision for CoC fracture, ceramic third bodies can lead to runaway wear of cobalt chrome (CoCr) causing extremely elevated blood cobalt. We present five cases of ceramic liner fractures revised to a CoCr head associated with the rapid development of severe cobalt toxicity. We identified 5 cases of fractured CoC THA treated with revision to CoCr on highly cross-linked polyethylene (HXLPE) – three to conventional bearings and two to modular dual mobility bearings (CoCr acetabular liner, CoCr femoral head, and HXLPE). Mean follow up was 2.5 years after CoCr/HXLPE re-revision. Symptoms of cobalt toxicity occurred at average 9.5 months after revision for ceramic fracture (range 6–12). All patients developed vision and hearing loss, balance difficulties, and peripheral neuropathy. Several had cardiomyopathy, endocrinopathy, and local skin discoloration. Two reported hip pain. Re-revision for cobalt toxicity occurred at an average of 22 months (range 10–36) after revision for ceramic fracture. Average serum cobalt level at re-revision was 991 μg/L (range 734–1302, normal <1 μg/L). All CoCr heads exhibited massive wear with asphericity; deep tissues exhibited prominent metallosis. Treatment consisted of debridement and revision to a ceramic head with HXLPE. Serum cobalt improved to an average of 25 μg/L at final follow up. All patients reported partial improvement in vision and hearing; peripheral neuropathy and balance did not recover. Systemic cobalt toxicity is a rare but devastating complication of ceramic fracture in THA treated with cobalt-alloy bearings. Cobalt alloy bearings should be avoided in this setting. The diagnosis of systemic cobalt toxicity requires a high index of suspicion and was typically delayed following systemic symptoms. Debridement and revision to a ceramic-on-HXLPE leads to improvement but not resolution of cobalt toxicity complications

Modular dual mobility (DM) articulations are increasingly utilized during total hip arthroplasty (THA). However, concerns remain regarding the metal liner modularity. This study aims to correlate metal artifact reduction sequence (MARS) magnetic resonance imaging (MRI) abnormalities with serum metal ion levels in patients with DM articulations. All patients with an asymptomatic, primary THA and DM articulation with >2-year follow-up underwent MARS-MRI of the operative hip. Each patient had serum cobalt, chromium, and titanium levels drawn. Patient satisfaction, Oxford Hip Score, and Forgotten Joint Score-12 (FJS-12) were collected. Each MARS-MRI was independently reviewed by fellowship-trained musculoskeletal radiologists blinded to serum ion levels. Forty-five patients (50 hips) with a modular DM articulation were included with average follow-up of 3.7±1.2 years. Two patients (4.4%) had abnormal periprosthetic fluid collections on MARS-MRI with cobalt levels >3.0 μg/L. Four patients (8.9%) had MARS-MRI findings consistent with greater trochanteric bursitis, all with cobalt levels < 1.0 μg/L. A seventh patient had a periprosthetic fluid collection with normal ion levels. Of the 38 patients without MARS-MRI abnormalities, 37 (97.4%) had cobalt levels <1.0 μg/L, while one (2.6%) had a cobalt level of 1.4 μg/L. One patient (2.2%) had a chromium level >3.0 μg/L and a periprosthetic fluid collection. Of the 41 patients with titanium levels, five (12.2%) had titanium levels >5.0 μg/L without associated MARS-MRI abnormalities. Periprosthetic fluid collections associated with elevated serum cobalt levels in patients with asymptomatic dual mobility articulations occur infrequently (4.4%), but further assessment of these patients is necessary. Aims. To report our experience with trunnion corrosion following metal-on-polyethylene total hip arthroplasty, in particular to report the spectrum of presentation and determine the mean time to presentation. Patients and Methods. We report the presenting symptoms and signs, intraoperative findings, and early results and complications of operative treatment in nine patients with a mean age of 74 years (60 to 86). The onset of symptoms was at a mean of seven years (3 to 18) after index surgery. Results. Patients presented with a variety of symptoms including pain, limp and rash. The preoperative mean serum cobalt level was 7.1 ppb (2.2 to 12.8) and mean serum chromium level was 2.2 ppb (0.5 to 5.2). Metal artifact reduction sequence (MARS) MRI showed fluid collection and possible pseudotumour formation in five hips, fluid collection in two hips, and synovitis/debris in one hip, with no MRI in one patient. Acetabular revision was performed in three patients, six patients underwent liner and head exchange only. The postoperative metal levels decreased in all patients: mean cobalt 0.5 ppb (0 to 1.8) and mean chromium 0.9 ppb (0 to 2.6) at a mean of five months (3 to 8) postoperatively. Seven patients had good pain relief and no complications at one year. There were two major complications requiring reoperation: acute infection at six weeks, for which the patient required two-stage reimplantation; and recurrent dislocation, for which the patient was revised to a dual mobility component. Conclusion. Trunnion corrosion in metal-on-polyethylene THA has a range of presenting symptoms, and may present later than previously described. A high index of suspicion is warranted, and serum cobalt and chromium levels are recommended for diagnosis. Patients should be counselled about possible postoperative complications. Recently, corrosion at the head-neck junction in metal-on-polyethylene bearing total hip arthroplasty (THA) has been recognized as a cause of adverse local tissue reactions (ALTR). Serum metal levels have been advocated as a tool for the diagnosis of ALTR, however no prior studies have specifically examined their utility. The purpose of this study was to determine the optimal cut-off values for serum cobalt and chromium in diagnosing ALTR after metal-on-polyethylene bearing THA. We reviewed 447 consecutive patients with serum metal levels tested at our institution and identified 62 with a metal-on-polyethylene bearing who had axial imaging or underwent reoperation to confirm the presence or absence of ALTR. Receiver operating characteristic curves were produced to identify cut-off thresholds to optimize sensitivity and diagnostic test performance was characterized. 42 Of the 62 patients (66%) were positive for an ALTR. The best test for the diagnosis of ALTR was the serum cobalt level (area under the curve [AUC]=99%). A threshold cut-off of ≥ 1.0 ng/ml had a sensitivity of 100%, specificity of 90%, positive predictive value (PPV) of 96%, and negative predictive value (NPV) of 100%. Serum chromium levels were also diagnostic (AUC=87%). A threshold cut-off of ≥ 0.15 ng/ml had a sensitivity of 100%, specificity of 50%, PPV of 81%, and NPV of 100%. Finally, serum cobalt to chromium ratio was also helpful for diagnosis (AUC=90%). A threshold cut-off of 1.4 for the cobalt to chromium ratio offered a sensitivity of 93%, specificity of 70%, PPV of 87%, and NPV of 82%. Measurement of serum cobalt with a threshold value of 1.0 ng/ml in our experience is the best test for identifying the presence of ALTR in patients with a metal-on-polyethylene THA. Measurement of chromium and the ratio of cobalt to chromium are also of value

We previously reported the five to ten-year results of the Birmingham Hip Resurfacing (BHR) implant. The purpose of this study was to evaluate the survivorship, radiographic results, and clinical outcomes of the BHR at long-term follow-up. We retrospectively reviewed 250 patients from the original cohort of 324 BHRs performed from 2006 to 2013 who met contemporary BHR indications. Of these, 4 patients died and 4 withdrew. From the 242 patients, 224 patients (93%) were available for analysis. Modified Harris hip score (mHHS) and University of California, Los Angeles (UCLA) scores were collected and compared to a matched total hip arthroplasty (THA) cohort. Mean follow-up was 14 years. Survivorship free of aseptic revision was 97.4% and survivorship free of any revision was 96.0% at 15 years. Revisions included 3 periprosthetic joint infections, 2 for elevated metal ions and symptomatic pseudotumor, 2 for aseptic femoral loosening, and 1 for unexplained pain. The mean mHHS was 93 in BHR patients at final follow-up, similar to the THA cohort (p=0.44). The UCLA score was significantly higher for BHR patients (p=0.02), however there were equal proportions of patients who remained highly active (UCLA 9 or 10) in both groups, 60.5% and 52.2% (p=0.45) for BHR and THA respectively. Metal ion levels at long term follow-up were low (mean serum cobalt 1.8±1.5 ppb and mean serum chromium 2.2±2.0 ppb). BHR demonstrated excellent survivorship in males less than 60 years of age at time of surgery. Clinical outcomes and activity levels were similar to THA patients. Failures related to the metal-on-metal bearing were rare and metal levels were low at long-term follow-up. Level of evidence: III. Keywords: survivorship; hip arthroplasty; activity; metal-on-metal. In a double-blinded randomised controlled trial, 83 patients with primary osteoarthritis of the hip received either a ceramic-on-metal (CoM) or metal-on-metal (MoM) total hip replacement (THR). The implants differed only in the bearing surfaces used. The serum levels of cobalt and chromium and functional outcome scores were compared pre-operatively and at six and 12 months post-operatively. Data were available for 41 CoM and 36 MoM THRs (four patients were lost to follow-up, two received incorrect implants). The baseline characteristics of both cohorts were similar. Femoral head size measured 36 mm in all but two patients who had 28 mm heads. The mean serum cobalt and chromium levels increased in both groups, with no difference noted between groups at six months (cobalt p = 0.67, chromium p = 0.87) and 12 months (cobalt p = 0.76, chromium p = 0.76) post-operatively. Similarly, the mean Oxford hip scores, Western Ontario and McMaster Universities Osteoarthritis index and University of California, Los Angeles activity scores showed comparable improvement at 12 months. Our findings indicate that CoM and MoM couplings are associated with an equivalent increase in serum cobalt and chromium levels, and comparable functional outcome scores at six and 12-months follow-up.

As adverse events related to metal on metal hip arthroplasty have been better understood, there has been increased interest in toxicity related to the high circulating levels of cobalt ions. However, distinguishing true toxicity from benign elevations in cobalt levels can be challenging. The purpose of this review is to examine the use of cobalt alloys in total hip arthroplasty, to review the methods of measuring circulating cobalt levels, to define a level of cobalt which is considered pathological and to review the pathophysiology, risk factors and treatment of cobalt toxicity. To the best of our knowledge, there are 18 published cases where cobalt metal ion toxicity has been attributed to the use of cobalt-chromium alloys in hip arthroplasty. Of these cases, the great majority reported systemic toxic reactions at serum cobalt levels more than 100 μg/L. This review highlights some of the clinical features of cobalt toxicity, with the goal that early awareness may decrease the risk factors for the development of cobalt toxicity and/or reduce its severity. Take home message: Severe adverse events can arise from the release of cobalt from metal-on-metal arthroplasties, and as such, orthopaedic surgeons should not only be aware of the presenting problems, but also have the knowledge to treat appropriately. Metal-on-metal bearings for total hip replacement (THR) are becoming increasingly popular. Improved wear characteristics mean that these articulations are being inserted into younger patients in the form of THR and resurfacing procedures. This has led to concerns regarding potential carcinogenicity because of the increased exposure to metal ions that the procedure brings. We have studied the serum cobalt and chromium concentrations in patients who had primary, well-fixed Ring metal-on-metal THRs for more than 30 years. The levels of cobalt and chromium were elevated by five and three times, respectively compared with those in our reference groups. Metal-on-metal articulations appear to be the source of metal ions throughout the life of the prosthesis. In three patients who had undergone revision of a previous metal-on-metal THR to a metal-on-polyethylene replacement the levels of metal ions were within the normal range. The elevations of cobalt and chromium ions seen in our study were comparable with those in patients with modern metal-on-metal THRs

Introduction. Modular neck (MN) components in total hip arthroplasty (THA) offer intraoperative flexibility, but adverse local tissue reactions (ALTR) due to tribocorrosion at modular junctions are a potential complication of such designs. Serum ion levels and metal artifact reduction sequence (MARS) MRI are used to assess ALTR following modular THA. This study investigates serum ion levels and MARS MRI findings in a series of hips with MN components and differing articulating surfaces. Methods. We retrospectively evaluated a cohort of 184 primary THAs in 159 patients implanted with a dual modular femoral stem by one surgeon from 2005–2013. 121 THAs had a cobalt-chromium neck component and non-metal-on-metal articulation, while 63 THAs had a titanium neck component and metal-on-metal (MoM) articulation. Serum ion levels were recorded for all patients. MARS MRI scans were read by musculoskeletal-trained radiologists. Pseudotumor grade and location were measured. Results. Serum cobalt levels as a function of time post-operatively demonstrated no correlation. In THAs with non-MoM articulation, pseudotumors were visualized in 13 of 54 (24.1%) initial MARS MRIs performed 1–40 months post-operatively compared to 5 of 67 (7.5%) performed 41–120 months post-operatively (p=0.02), and findings typically remained consistent across multiple MARS MRI scans. Pseudotumors were generally located lateral to the hip joint in hips with non-MoM articulation compared to anteromedial following MoM arthroplasty. In the cobalt-chrome MN group, cobalt levels were elevated in 11 of 20 (55%) of patients with pseudotumor compared to 15 of 19 (79%) in the MoM group. Conclusion. Generation of ALTR leading to pseudotumor formation is a concern with cobalt-chrome MN designs. Psuedotumor characteristics differ between patients with cobalt-chrome designs versus MoM articulation. Normal serum ion levels did not exclude the presence of a pseudotumor and routine MARS MRI should be included in follow-up of patients with cobalt-chromium MN prostheses

Background. Adverse local tissue reactions (ALTR) in metal-on-polyethylene (MoP) total hip arthroplasty (THA) with head-neck taper corrosion is likely to be multifactorial involving implant and patient factors. However, there is a paucity of clinical data on implant parameters as predisposing factors in MoP head-neck taper corrosion. The aim of this study was to identify any potential implant factors associated with failed MoP THA due to head-neck taper corrosion. Methods. A total of 67 MoP THA patients in two groups was investigated: 1) ALTR (n=38) on MARS MRI and 2) non-ALTR (n=29) on MARS MRI. All patients had highly cross-linked polyethylene liners with cobalt-chromium femoral heads with a single head-neck modularity. Parameters compared between groups included: acetabular component orientation, femoral neck shaft angle, radiographic measurement of medial and vertical femoral offsets, limb length discrepancy, component size, femoral head offset, implant type, femoral stem alloy and taper design. Results. The occurrence of ALTR was associated with increased femoral head offset (non-ALTR vs. ALTR, 0.5 ± 3.7 vs. 4.1 ± 3.6, p = 0.008), increased radiographic femoral stem offset (35.9 ± 7.4 mm vs 40.7 ± 7.1mm, p = 0.018), and the use of TMZF alloy stems (p = 0.051). The presence of ALTR was significantly associated with higher cobalt (9.1 µg/L vs. 0.4µg/L, p=0.001) and chromium (2.6µg/L vs. 0.4µg/L). A significant correlation was identified between the serum cobalt level and the femoral stem offset (R=0.375, p=0.009). Conclusions. Although the occurrence of head-neck taper corrosion in MoP THA is likely to be an interplay between implant and patient factors, the results of this study identified increased femoral head and stem offset and the use of TMZF alloy stems as risk factors associated with clinically relevant ALTR due to head-neck taper corrosion in MoP THA patients. Aims . The primary aim of this independent prospective randomised trial was to compare serum metal ion levels for ceramic-on-metal (CoM) and metal-on-metal (MoM) bearing surfaces in total hip arthroplasty (THA). Our one-year results demonstrated elevation in metal ion levels above baseline with no significant difference between the CoM and MoM groups. This paper reviews the five-year data. Patients and Methods. The implants used in each patient differed only in respect to the type of femoral head (ceramic or metal). At five-year follow-up of the 83 enrolled patients, data from 67 (36 CoM, 31 MoM) was available for comparison. Results. The mean serum cobalt (Co) and chromium (Cr) ion levels remained above baseline in both groups (CoM: Co 1.16 μg/l (0.41 to 14.67), Cr 1.05 μg/l (0.16 to 12.58); MoM: Co 2.93 μg/l (0.35 to 30.29), Cr 1.85 μg/l (0.36 to 17.00)) but the increase was significantly less in the CoM cohort (Co difference p = 0.001, Cr difference p = 0.002). These medium-term results, coupled with lower revision rates from national joint registries, suggest that the performance of CoM THA may be superior to that of MoM. . Conclusion. While both bearing combinations have since been withdrawn these results provide useful information for planning clinical surveillance of CoM THAs and warrants continued monitoring. Trunnion corrosion in metal-on-polyethylene THA is poorly understood, with multifactorial etiology, and the patients present with “hip pain”. We analysed the presenting symptoms and signs, intraoperative findings and the early results and complications of operative treatment. One surgeon treated 9 patients (6 male, 3 female), mean age 74 years, with the onset of symptoms at a mean of 7 years (range 3–18) after index surgery. The taper size was 12/14 in seven, 14/16 in one, and 6 degree in one hip. The preoperative mean cobalt level was 7.1 ppb (range, 2.2–12.8) and mean chromium level was 2.2 ppb (range, 0.5–5.2). MARS MRI showed fluid collection and pseudotumor in 5, fluid collection only in two, and synovitis/debris in one hip. In one patient, there was no preoperative MRI. There were a myriad of clinical presentations: thigh rash alone in one; diffuse leg pain and hip rash in one; acute pseudo-sepsis in one; iliopsoas tendinitis and diffuse rash in one; trochanteric bursitis in one; groin pain only in one; thigh-buttock pain in two; and diffuse hip pain and limp in one patient. Intraoperatively, 6 patients had liner and ceramic (or oxidized zirconium) head exchange only. Three patients had concurrent acetabular revision: one for broken locking mechanism; one because liner was unavailable, and one had acetabular loosening. The postoperative metal levels decreased in all patients: mean cobalt 0.5 ppb (range, 0–1.8) and mean chromium 0.9 ppb (range, 0–2.6). Seven patients had good pain relief and no complications. There were two major complications requiring reoperation: acute infection at 6 weeks and patient required 2-stage reimplantation; and second patient had recurrent dislocation and was revised to a dual mobility component. Trunnion corrosion in metal-on-polyethylene THA has several clinical presentations, including local skin rash, iliopsoas tendinitis, and other limb dysfunction. There should be a high index of suspicion and serum cobalt/chromium levels are recommended for diagnosis. The patients should be counseled about possible postoperative complications

Introduction. Because of concerns regarding excessive wear and short-term failures attributed to the metal-on-metal bearings, the use of metal-on-metal hip resurfacing arthroplasty (MOMHRA) has been greatly reduced since 2008, despite great mid-term results for well-designed implants and in certain patient populations. The true cause of excessive wear was then unknown. Therefore, identification of true risk factors for the procedure became paramount to refine indications and improve survivorship outcomes. Methods. Over the last 10 years, a systematic search of the US national library of Medicine and National institutes of health with the key words “metal-on-metal” and hip resurfacing” was conducted and returned 2186 items. Of these items, 862 were deemed relevant to our research purposes and entered in our center's reference database from which this review was performed. Results. Edge loading is the main culprit for high wear and high serum Co and Cr ion concentrations because it disrupts the fluid film lubrication of the device. Computation of the contact patch to rim distance (CPR), an estimate of the joint's functional coverage, is the best predictor of potential edge loading and excessive wear. Both in vivo and in vitro studies show that the wear of well-designed and well-positioned MOM bearings diminishes over time with continued use, an advantage only featured by MOM bearings. Systemic wear-related complications and hypersensitivity to metal once thought to be common are in fact rare occurrences. In addition, metal-related revisions only represent a small portion of the various modes of failure encountered with well-designed HRA. In our series of 1321 hips with only 0.5% lost to follow-up, 11 patients underwent revision surgery for excessive wear or adverse local tissue reaction. All but 2 had mal-positioned acetabular components (CPR distance <10mm). One of these 2 patients had serum cobalt and chromium levels of 13 and 9 µg/L respectively, despite a CPR distance of 18.3 mm, while the other showed a peri-prosthetic fluid collection estimated at 111cc on MRI. Component aseptic loosening (acetabular or femoral) remains, as is also the case for total hip arthroplasty (THA), the leading indication for revision surgery, even though substantial progress has been reported to reduce its incidence. Femoral neck fractures and loosening are associated with the surgeon's learning curve which can be avoided with proper training. The survivorship of the femoral component in our series after implementation of surgical technique changes is 99.2% at 10 years for an overall survivorship of 95.5%. A diagnosis of DDH is associated with a higher failure rate, particularly from socket loosening, while women without risk factors have a survivorship of 98.6% at 15 years. Conclusion. HRA has often been, but should not be associated with the results of large head metal-on-metal THA which often present with a different set of failure mechanisms. Now that most of the risk factors for HRA have been identified, and solutions found, a balanced perspective of its results is needed because the long-term data available validate the low wear of MOM bearings which was predicted in the early hip simulator studies. Considering the tremendous progress made in surgical technique and the advanced tribological knowledge acquired with the last 20 years of investigations related to MOM HRA, all the conditions are present to make this procedure a success for lifetime durability as shown in our series where 44 patients (54 hips) have died without a revision at a mean follow-up time of 9.7 years (range, 1.8 to 19.7). For HRA, when devices with adequate coverage and clearance of the ball by the socket are used, severe developmental dysplasia and inadequate surgical technique are responsible for most failures at 10 to 15 years of follow-up. It is our opinion that the advantages of HRA over THA (such as absence of thigh pain, a low dislocation rate, no taper corrosion, and the preservation of proximal femoral bone mineral density and the ability to maintain high activity levels without penalty) now outweigh the risks of using a MOM bearing

There is widespread concern regarding the incidence of adverse soft-tissue reactions after metal-on-metal (MoM) hip replacement. Recent National Joint Registry data have shown clear differences in the rates of failure of different designs of hip resurfacing. Our aim was to update the failure rates related to metal debris for the Articular Surface Replacement (ASR). A total of 505 of these were implanted. Kaplan-Meier analysis showed a failure rate of 25% at six years for the ASR resurfacing and of 48.8% for the ASR total hip replacement (THR). Of 257 patients with a minimum follow-up of two years, 67 (26.1%) had a serum cobalt concentration which was greater than 7 μg/l. Co-ordinate measuring machine analysis of revised components showed that all patients suffering adverse tissue reactions in the resurfacing group had abnormal wear of the bearing surfaces. Six THR patients had relatively low rates of articular wear, but were found to have considerable damage at the trunion-taper interface. Our results suggest that wear at the modular junction is an important factor in the development of adverse tissue reactions after implantation of a large-diameter MoM THR

Aims

In 2015, we published the results of our ceramic-on-metal (CoM) total hip arthroplasties (THAs) performed between October 2007 and July 2009 with a mean follow-up of 34 months (23 to 45) and a revision rate of 3.1%. The aim of this paper is to present the longer-term outcomes.

Methods

A total of 264 patients were reviewed at a mean of 5.8 years (4.6 to 7.2) and 10.1 years (9.2 to 10.6) to determine revision rate, pain, outcome scores, radiological analysis, and blood ion levels. Those who were unwilling or unable to travel were contacted by telephone.
