The attitudes of orthopaedic surgeons regarding radiology reporting is not well-described in the literature. We surveyed Orthopaedic Surgeons in Australia and New Zealand to assess if they routinely review formal radiology reports. An anonymized, 14 question online survey was distributed to consultant surgeons of the Australian and New Zealand Orthopaedic Associations (AOA, NZOA). Two hundred respondents completed the survey (Total number of Fellows: 283 NZOA, 1185 AOA). 18.5% of respondents always reviewed the formal Radiology report, 44.5% most of the time, 35% sometimes and 2% never. By imaging modality, MRI reports were the most frequently reviewed (92%), followed by ultrasound (74%) and nuclear medicine (63%). Only 10% of surgeons consulted formal reports for plain radiography. 55% of surgeons were still likely to disagree with the MRI report, followed by 46% for plain radiography. In cases of disagreement, only 21% of surgeons would always contact the reporting radiologist. The majority of Surgeons (85.5%) think there should be more collaboration between the disciplines, although only 50.5% had regular attendance of a Radiologist at their departmental audit. This survey reveals that the majority of orthopaedic surgeons are not routinely reading radiology reports. This points towards a need for further interdisciplinary collaboration. To our knowledge, this is the first survey directly assessing attitudes of orthopaedic surgeons towards radiology reports

Introduction. Derotation osteotomies are commonly performed in paediatric orthopaedic and limb reconstruction practice. The purpose of this study was to determine whether the use of a digital inclinometer significantly improves the accuracy in attaining the desired correction. Materials & Methods. We designed an electronic survey regarding derotation femoral osteotomy (DFO) including methods of intra-operative angular correction assessment and acceptable margins of error for correction. This was distributed to 28 paediatric orthopaedic surgeons in our region. A DFO model was created, using an anatomic sawbone with foam covering. 8 orthopaedic surgeons each performed two 30-degree DFOs, one using K-wires and visual estimation (VE), and the other using a Digital Inclinometer (DI). Two radiologists reported pre and post procedure rotational profile CT scans to assess the achieved rotational correction. Results. There was a 68% response rate to the survey. The most popular methods of estimating intra-operative correction were reported to be K-wires and rotation marks on bone. The majority of respondents reported that a 6–10 degree margin of error was acceptable for a 30-degree derotation. This was therefore set as the upper limit for acceptable error margin in the simulation study. The mean error in rotation in the VE group of simulated DFO was 19.7 degrees, with error>5 degrees and error>10 degrees in 7 (88%) and 6 (75%) cases respectively. Mean error in DI group was 3.1 degrees, with error>5 degrees in 1 case (13%). Conclusions. Our results show that the compared to conventional techniques, the use of an inclinometer significantly improves the accuracy of femoral de-rotation and significantly reduces the incidence of unacceptable errors in correction. We would suggest that digital inclinometers be used to assess intra-operative correction during derotation osteotomies

Abstract. Purpose. Intracanal rib head penetration is a well-known entity in dystrophic scoliotic curves in neurofibromatosis type 1. There is potential for spinal cord injury if this is not recognised and managed appropriately. No current CT-based classification system is currently in use to quantify rib head penetration. This study aims to propose and evaluate a novel CT-based classification for rib head penetration primarily for neurofibromatosis but which can also be utilised in other conditions of rib head penetration. Materials and methods. The grading was developed as four grades: normal rib head (RH) position—Grade 0, subluxed ext-racanal RH position—Grade 1, RH at pedicle—Grade 2, intracanal RH—Grade 3. Grade 3 was further classified depending on the head position in the canal divided into thirds. Rib head penetration into proximal third (from ipsilateral side)—Grade 3A, into the middle third—Grade 3B and into the distal third—Grade 3C. Seventy-five axial CT images of Neurofibromatosis Type 1 patients in the paediatric age group were reviewed by a radiologist and a spinal surgeon independently to assess interobserver and intraobserver agreement of the novel CT classification. Agreement analysis was performed using the weighted Kappa statistic. Results. There was substantial interobserver correlation with mean Kappa score (k = 0.8, 95% CI 0.7–0.9) and near perfect intraobserver Kappa of 1.0 (95% CI 0.9–1.0) and 0.9 (95% CI 0.9–1.0) for the two readers. Conclusion. The novel CT-based classification quantifies rib head penetration which aids in management planning

The diagnosis of infection following shoulder arthroplasty is notoriously difficult. The prevalence of prosthetic shoulder infection after arthroplasty ranges from 3.9 – 15.4% and the most common infective organism is Cutibacterium acnes. Current preoperative diagnostic tests fail to provide a reliable means of diagnosis including WBC, ESR, CRP and joint aspiration. Fluoroscopic-guided percutaneous synovial biopsy (PSB) has previously been reported in the context of a pilot study and demonstrated promising results. The purpose of this study was to determine the diagnostic accuracy of percutaneous synovial biopsy compared with open culture results (gold standard). This was a multicenter prospective cohort study involving four sites and 98 patients who underwent revision shoulder arthroplasty. The cohort was 60% female with a mean age was 65 years (range 36-83 years). Enrollment occurred between June 2014 and November 2021. Pre-operative fluoroscopy-guided synovial biopsies were carried out by musculoskeletal radiologists prior to revision surgery. A minimum of five synovial capsular tissue biopsies were obtained from five separate regions in the shoulder. Revision shoulder arthroplasty was performed by fellowship-trained shoulder surgeons. Intraoperative tissue samples were taken from five regions of the joint capsule during revision surgery. Of 98 patients who underwent revision surgery, 71 patients underwent both the synovial biopsy and open biopsy at time of revision surgery. Nineteen percent had positive infection based on PSB, and 22% had confirmed culture positive infections based on intra-operative tissue sampling. The diagnostic accuracy of PSB compared with open biopsy results were as follows: sensitivity 0.37 (95%CI 0.13-0.61), specificity 0.81 (95%CI 0.7-0.91), positive predictive value 0.37 (95%CI 0.13 – 0.61), negative predictive value 0.81 (95%CI 0.70-0.91), positive likelihood ratio 1.98 and negative likelihood ratio 0.77. A patient with a positive pre-operative PSB undergoing revision surgery had an 37% probability of having true positive infection. A patient with a negative pre-operative PSB has an 81% chance of being infection-free. PSB appears to be of value mainly in ruling out the presence of peri-prosthetic infection. However, poor likelihood ratios suggest that other ancillary tests are required in the pre-operative workup of the potentially infected patient

Bone age is a radiographical assessment used in pediatric medicine due to its relative objectivity in determining biological maturity compared to chronological age and size.1 Currently, Greulich and Pyle (GP) is one of the most common methods used to determine bone age from hand radiographs.2–4 In recent years, new methods were developed to increase the efficiency in bone age analysis like the shorthand bone age (SBA) and the automated artificial intelligence algorithms. The purpose of this study is to evaluate the accuracy and reliability of these two methods and examine if the reduction in analysis time compromises their accuracy. Two hundred thirteen males and 213 females were selected. Each participant had their bone age determined by two separate raters using the GP (M1) and SBA methods (M2). Three weeks later, the two raters repeated the analysis of the radiographs. The raters timed themselves using an online stopwatch while analyzing the radiograph on a computer screen. De-identified radiographs were securely uploaded to an automated algorithm developed by a group of radiologists in Toronto. The gold standard was determined to be the radiology report attached to each radiograph, written by experienced radiologists using GP (M1). For intra-rater variability, intraclass correlation analysis between trial 1 (T1) and trial 2 (T2) for each rater and method was performed. For inter-rater variability, intraclass correlation was performed between rater 1 (R1) and rater 2 (R2) for each method and trial. Intraclass correlation between each method and the gold standard fell within the 0.8–0.9 range, highlighting significant agreement. Most of the comparisons showed a statistically significant difference between the two new methods and the gold standard; however it may not be clinically significant as it ranges between 0.25–0.5 years. A bone age is considered clinically abnormal if it falls outside 2 standard deviations of the chronological age; standard deviations are calculated and provided in GP atlas.6–8 For a 10-year old female, 2 standard deviations constitute 21.6 months which far outweighs the difference reported here between SBA, automated algorithm and the gold standard. The median time for completion using the GP method was 21.83 seconds for rater 1 and 9.30 seconds for rater 2. In comparison, SBA required a median time of 7 seconds for rater 1 and 5 seconds for rater 2. The automated method had no time restraint as bone age was determined immediately upon radiograph upload. The correlation between the two trials in each method and rater (i.e. R1M1T1 vs R1M1T2) was excellent (κ= 0.9–1) confirming the reliability of the two new methods. Similarly, the correlation between the two raters in each method and trial (i.e. R1M1T1 vs R2M1T1) fell within the 0.9–1 range. This indicates a limited variability between raters who may use these two methods. The shorthand bone age method and an artificial intelligence automated algorithm produced values that are in agreement with the gold standard Greulich and Pyle, while reducing analysis time and maintaining a high inter-rater and intra-rater reliability

Abstract. Purpose. No clinical CT based classification system is currently in use for Lumbar Foraminal Stenosis. MRI scanners are not easily available, are expensive and may be contraindicated in an increasing number of patients. This study aims to propose and evaluate the reproducibility of a novel CT based classification for lumbar foraminal stenosis. Materials and Methods. The grading was developed as 4 grades. Normal foramen – Grade 0, Anteroposterior(AP)/Superoinferior (SI)(single plane) fat compression – Grade 1, Both AP/SI compression (two planes) – Grade 2 (both AP and SI) without distortion of nerve root, Grade 2 with distortion of nerve root – Grade 3. 800 lumbar foramen of a cohort of 100 random patients over the age of 60 who had undergone both CT and MRI scans were reviewed by two radiologists independently to assess agreement of the novel CT classification against the MRI based grading system of Lee et al. Interobserver(n=400) and intraobserver agreement(n=160) was also evaluated. Agreement analysis was performed using the Weighted Kappa statistic. Results. 100 patients (M:F = 45:55) with a mean age of 68.5 years (range 60 – 83 years were included in the study. The duration between CT and MRI scans was 98 days(range 0 – 540, SD – 108). There was good correlation between CT and MRI with Kappa scores (k=0.81) and intraobserver Kappa of 0.89 and 0.98 for the two readers. Conclusion. The novel CT based classification correlates well with the MRI grading system and can safely and accurately replace it where required

Ceramic-on-ceramic bearings provide a solution to the osteolysis seen with traditional metal-on-polyethylene bearings. Sporadic reports of ceramic breakage and squeaking concern some surgeons and this bearing combination can show in vivo signs of edge loading wear which was not predicted from in vitro studies. Taper damage or debris in the taper between the ceramic and metal may lead to breakage of either a ceramic head or insert. Fastidious surgical technique may help to minimise the risk of ceramic breakage. Squeaking is usually a benign complication, most frequently occurring when the hip is fully flexed. Rarely, it can occur with each step of walking when it can be sufficiently troublesome to require revision surgery. The etiology of squeaking is multifactorial in origin. Taller, heavier and younger patients with higher activity levels are more prone to hips that squeak. Cup version and inclination are also relevant factors. Osteolysis following metal-on-UHMW polyethylene Total Hip Arthroplasty (THA) is well reported. Earlier generation ceramic-on-ceramic bearings did produce some osteolysis, but in flawed implants. As third and now fourth generation ceramic THAs come into mid- and long-term service, the orthopaedic community has begun to see reports of high survival rates and very low incidence of osteolysis in these bearings. The technique used by radiologists for identifying the nature of lesions on Computed Tomography (CT) scan is the Hounsfield score which will identify the density of the tissue within the lucent area. Commonly the radiologist will have no access to previous imaging, especially pre-operative imaging if a long time has elapsed. With such a low incidence of osteolysis in this patient group, what, then, should a surgeon do on receiving a CT report on a ceramic-on-ceramic THA, which states there is osteolysis? This retrospective review aims to determine the accuracy of CT in identifying true osteolysis in a cohort of long-term third generation ceramic-on-ceramic uncemented hip arthroplasties in our department. Pelvic CT scans were performed on the first 27 patients from a cohort of 301 patients undergoing 15-year review with third generation alumina-alumina cementless THAs. The average follow-up was 15 years (15–17). The CT scans were reviewed against pre-operative and post-operative radiographs and reviewed by a second musculoskeletal specialist radiologist. Eleven of the CT scans were reported to show acetabular osteolysis, two reported osteolysis or a possible pre-existing cyst and one reported a definitive pre-existing cyst. After review of previous imaging including pre-operative radiographs, eleven of the thirteen patients initially reported to have osteolysis were found to have pre-existing cysts or geodes in the same size and position as the reported osteolysis, and a further patient had spot-welds with stress-shielding. One patient with evidence of true osteolysis awaits aspiration or biopsy to determine if he has evidence of ceramic wear or metallosis. Reports of osteolysis on CT should be interpreted with care in modern ceramic-on-ceramic THA to prevent unnecessary revision. Further imaging and investigations may be necessary to exclude other conditions such as geodes, or stress shielding which are frequently confused with osteolysis on CT scans

Background. It has been suggested that routine follow-up of primary THR patients could be performed by GPs in primary care, rather than by orthopaedic specialists. Essential radiographic follow-up would likely be based on radiographic reports, rather than on inspection of the radiographs themselves. Aim. To look at the quality of the radiographic reports to determine their usefulness as a method of radiographic follow-up of THRs. Methods. The immediate pre-revision radiographs of a consecutive series of 50 patients, who had undergone revision surgery in 2 hospitals, were examined by a senior orthopaedic registrar, supervised by a Consultant with a revision THR practice, and compared to the findings of the corresponding radiologist report. Signs of failure were categorised into loosening, bone lysis and polyethylene wear. Results. Comparison to previous radiographs, essential in assessing THRs, was only performed by radiologists in 42% of cases. Fixation type (cemented/uncemented) was mentioned in only 2 cases and was correct in 1 case. With regard to loosening, acetabular component loosening was missed by radiologists in 10% of cases and femoral component loosening in 12%. Radiologists missed osteolysis on the acetabular side in 28% of cases and on the femoral side in 26%. Polyethylene wear was reported by surgeons in 30% of cases, but radiologists failed to report this in 28% of cases. Discussion. The radiographs reviewed contained definite evidence of failure, being pre-revision radiographs. Despite this, a significant amount of important information was lacking in the radiologists' reports. Primary care doctors would have been reassured that all was well with the prosthesis in a significant number of cases and hence specialist referral would have not taken place, unless the patient was symptomatic. We cannot support the radiographic component of GP follow-up of primary THRs using radiological reports on the basis of this study

Fifty-five ceramic bearings revised at our center were collected over 12 years. Median time to revision was 2.7 years. Forty-six (84%) cases had edge loading wear. The median femoral head wear volume overall was 0.2mm. 3. /yr, for anterosuperior edge loading was 2.0mm. 3. /yr, and the median volumetric wear rate for posterior edge loading was 0.15mm. 3. /yr (p=0.005). Myxofibrosarcoma (MFS) is the second most common subtype of soft tissue sarcoma (STS) and is associated with a high rate of local recurrence after resection. These tumours frequently present with peri-lesional edema, termed “tumour tails” on staging MRI scans [1]. Tumour tails(TT) may contain satellite neoplastic cells or can represent benign reactive edema. There are no clear radiological features to distinguish malignant from reactive peri-lesional edema which limits accurate surgical planning, resulting in either high rates of inadvertently positive resection margins and local recurrences or overly-aggressive resections which negatively impact function and increase morbidity [2]. The objective of this pilot study was to prospectively study a cohort of MFS patients with TTs in an attempt to identify radiological features that predict which type of edema is malignant and requires resection together with the main tumour mass. Patients diagnosed with MFS on biopsy at an orthopaedic oncology referral centre between January 1-December 31 2018 who also had TTs on staging MRI scans were prospectively recruited for the study. Tumours were treated with wide surgical excision, including the TTs, and (neo)adjuvant radiotherapy as per institutional protocol. Staging MRI scans were reviewed in a blinded fashion by two musculoskeletal radiologists to distinguish malignant from reactive TTs. The main tumour mass underwent standard histological evaluation while the regions encompassing the TTs were photographed and sectioned into grids. Each tissue section was examined histologically for the presence of satellite neoplastic cells based on morphological criteria. Radiological and histological findings were compared. Six patients met the inclusion criteria and underwent analysis. All tumours were located in the extremities and were deep to fascia. Mean age at presentation was 67 years (range 51 – 85), with a male:female ratio of 4:2. All patients received radiotherapy (50 Gy), either pre- (n=4) or post-operatively (n=2) based on multidisciplinary tumor board discussion or enrolment in a prospective clinical trial. Radiologically, TTs were labelled as malignant in four patients (66.7%) and as benign TTs in two others. The tails were recognised to be malignant due to the differing signal characteristics to reactive edema on mixed MRI sequences. The radiological evaluation correlated exactly with histological analysis, as satellite neoplastic cells were identified microscopically in the same four cases in which the TTs were designated to be malignant by MRI (specificity&sensitivity=100%). Surgical resection margins were microscopically positive in 50% of cases in the TTs themselves, and 75% of cases in which TTs were designated as malignant on staging MRI. “The malignant nature of peri-lesional edema in MFS, also known as the TT, was accurately predicted in this small pilot study based on specific radiological features which correlated exactly with histologic identification of isolated tumor cells. These findings validate development of a larger prospective study to recruit additional patients with tumor tails beyond just MFS, in order to more robustly study the correlation between the MRI appearance and histological distribution of satellite sarcoma cells in peri-lesional edema in STS. We are already recruiting to this expanded radiological-histological investigation including evaluation of additional novel MRI sequences

Ultrasound (US) is the standard imaging modality used to screen for developmental dysplasia of the hip in infants. Currently, radiologists or orthopaedic surgeons review scan images and judge them to be adequate or inadequate for interpretation. If considered adequate, diagnostic dysplasia metrics are determined; however, there is no standardised method for this process. There is significant inter-observer variability in this manual process which can affect misdiagnosis rates. To eliminate this subjectivity, we developed an automatic method to identify adequate US images and extract dysplasia metrics. The purpose of this study was to validate the efficacy of this automatic method by comparing results with observer-determined dysplasia metrics on a set of US images. A total of 693 US images from scans of 35 infants were analysed. Trained clinicians at a single institution labeled each image as adequate or inadequate, and subsequently measured alpha and beta angles on adequate images to diagnose dysplasia. We trained our image classifier on random sets of 415 images and used it to assess the remaining 278 images. Alpha and beta angles were automatically estimated on all adequate images. We compared the manual and automatic methods for discrepancies in adequacy determination, metric variability and incidences of missed early diagnosis or over-treatment. There was excellent agreement between the automatic and manual methods in image adequacy classification (Kappa coefficient = 0.912). On each adequate US image, alpha and beta angle measurements were compared, producing mixed levels of agreement between methods. Mean discrepancies of 1.78°±4.72° and 8.91°±6.437° were seen for alpha and beta angles, respectively. Standard deviations of the angle measures across multiple images from a single patient scan were significantly reduced by the automatic method for both alpha (p<0.05) and beta (p<0.01) angles. Additionally, the automatic method classified three hips (two patients) as Graf type II and two hips (two patients) as type III, while the manual method classified them as type I and II, respectively. Both cases flagged as type III patients by the automatic system subsequently failed Pavlik harness treatment and were booked for surgery. The automatic method produced excellent agreement with radiologists in scan adequacy classification and significantly reduced measurement variability. Good agreement between methods was found in Graf classification. In instances of disagreement, subsequent clinical findings seemed to support the classification of the automatic method. This proposed method presents an alternative automatic, near-real-time analysis for US images that may potentially significantly improve dysplasia metric reliability and reduce missed early diagnoses without increasing over-treatment

Introduction. In the evaluation of patients with pre-arthritic hip disorders, making the correct diagnosis and identifying the underlying bone pathology is of upmost importance to achieve optimal patient outcomes. 3-dimensional imaging adds information for proper preoperative planning. CT scans have become the gold standard for this, but with the associated risk of radiation exposure to this generally younger patient cohort. Purpose. To determine if 3D-MR reconstructions of the hip can be used to accurately demonstrate femoral and acetabular morphology in the setting of femoroacetabular impingement (FAI) and development dysplasia of the hip (DDH) that is comparable to CT imaging. Materials and Methods. We performed a retrospective review of 14 consecutive patients with a diagnosis of FAI or DDH that underwent both CT and MRI scans of the same hip with 3D reconstructions. 2 fellowship trained musculoskeletal radiologists reviewed all scans, and a fellowship trained hip preservation surgeon separately reviewed scans for relevant surgical parameters. All were blinded to the patients' clinical history. The 3D reconstructions were evaluated by radiologists for the presence of a CAM lesion and acetabular retroversion, while the hip preservation surgeon also evaluated CAM extent using a clock face convention of a right hip, location of femoral head blood supply, and morphological anterior inferior iliac spine (AIIS) variant. The findings on the 3D CT reconstructions were considered the reference standard. Results. Of 14 patients, there were 9 females and 5 males with a mean age 32 (range 15–42). There was no difference in the ability of MRI to detect the presence of a CAM lesion (100% agreement between 3D-MR and 3D-CT, p=1), AIIS morphology (p=1, mode=type 1 variant), or acetabular retroversion (85.7%, p=0.5). 3D-MR had a sensitivity and specificity of 100 in detecting a CAM lesion relative to 3D-CT. Four CT studies were inadequate to adequately evaluate for presence of a CAM. Five CT studies were inadequate to evaluate for location of the femoral head vessels, while MRI was able to determine location in those patients. In the 10 remaining patients for presence of CAM, and nine patients for femoral head vessel location, there was no statistically significant difference between 3D-MR and 3D-CT in determining the location of CAM lesion on a clock face (p=0.8, mean MRI = 12:54, mean CT: 12:51, SD = 66 mins MR, 81 mins CT) or in determining vessel location (p=0.4, MR mean 11:23, CT mean 11:36, SD 33 mins for both). Conclusion. 3D MRI reconstructions are as accurate as 3D CT reconstructions in evaluating osseous morphology of the hip, and may be superior to CT in determining other certain clinically relevant hip parameters. 3D-MR was equally useful in determining the presence and extent of a CAM lesion, acetabular retroversion, and AIIS morphologic variant, and more useful than 3D CT in determining location of the femoral head vessels. In evaluating FAI or hip dysplasia, a 3D-MR study is sufficient to evaluate both soft tissue and osseous anatomy, sparing the need for a 3D CT scan and its associated radiation exposure and cost

Surgical reattachment of torn rotator cuff tendons can lead to satisfactory clinical outcome but failures remain common. Ortho-R product is a freeze-dried formulation of chitosan (CS) that is solubilized in platelet-rich plasma (PRP) to form injectable implants. The purpose of the current pilot study was to determine Ortho-R implant acute residency, test safety of different implant doses, and assess efficacy over standard of care in a sheep model. The infraspinatus tendon (ISP) was detached and immediately repaired in 22 skeletally mature ewes. Repair was done with four suture anchors in a suture bridge configuration (n = 6 controls). Freeze-dried formulations containing 1% w/v chitosan (number average molar mass 35 kDa and degree of deacetylation 83%) with 1% w/v trehalose (as lyoprotectant) and 42.2 mM calcium chloride (as clot activator) were solubilized with autologous leukocyte-rich PRP and injected at the tendon-bone interface and on top of the repaired site (n = 6 with a 1 mL dose and n = 6 with a 2 mL dose). Acute implant residency was assessed histologically at 1 day (n = 2 with a 1 mL dose and n = 2 with a 2 mL dose). Outcome measures included MRI assessment at baseline, 6 weeks and 12 weeks, histopathology at 12 weeks and clinical pathology. MRI images and histological slides were scored by 2 blinded readers (veterinarian and human radiologist, and veterinarian pathologist) and averaged. The Generalized Linear Model task (SAS Enterprise Guide 7.1 and SAS 9.4) was used to compare the different groups with post-hoc analysis to test for pairwise differences. Ortho-R implants were detected near the enthesis, near the top of the anchors holes and at the surface of ISP tendon and muscle at 1 day. Numerous polymorphonuclear cells were recruited to the implant in the case of ISP tendon and muscle. On MRI, all repair sites were hyperintense compared to normal tendon at 6 weeks and only 1 out 18 repair sites was isointense at 12 weeks. The tendon repair site gap seen on MRI, which is the length of the hyperintense region between the greater tuberosity and tendon with normal signal intensity, was decreased by treatment with the 2 mL dose when compared to control at 12 weeks (p = 0.01). Histologically, none of the repair sites were structurally normal. A trend of improved structural organization of the tendon (p = 0.06) and improved structural appearance of the enthesis (p = 0.1) with 2 mL dose treatment compared to control was seen at 12 weeks. There was no treatment-specific effect on all standard safety outcome measures, which suggests high safety. Ortho-R implants (2 mL dose) modulated the rotator cuff healing processes in this large animal model. The promising MRI and histological findings may translate into improved mechanical performance, which will be assessed in a future study with a larger number of animals. This study provides preliminary evidence on the safety and efficacy of Ortho-R implants in a large animal model that could potentially be translated to a clinical setting

Introduction. Total knee arthroplasty (TKA) is one of the most successful surgeries to relieve pain and dysfunction caused by severe arthritis. However, it is a still big problem that there is a possibility of death in pulmonary embolism (PE) after TKA. We previously reported that there was more incidence of asymptomatic PE than estimated in general by detail examinations. But it was difficult to whom we decided to perform additional examinations except the patients with some doubtful symptoms. Therefore, we investigated detail of PE patients after primary TKA to find out anything key point in PE. Methods. Consecutive ninety-nine patients who underwent primary TKA from January 2015 to March 2018 were applied. There were 23 male and 76 females included, and the mean age was 73.7 years old. There were 96 cases of osteoarthritis, 2 cases of osteonecrosis and one of rheumatoid arthritis. A single knee surgery team performed all operations with cemented type prostheses and air tourniquet during operation. There were 35 cases of one-staged bilateral TKA and 64 of unilateral TKA. Detail examinations with contrast enhanced CT (CE-CT) and venous ultrasonography (US) were performed at the 3rd day after surgery. Next, we applied ultra sound cardiogram (UCG) to the patients diagnosed as PE by CE-CT, we checked right ventricular overload (RVO) to treat PE. These images were read by a single senior radiologist team. Results. CE-CT was performed in 87 patients (87.8%) and US was in all patients. The incidence of deep vein thrombosis (DVT) was 67 patients (67.6%), and all of these was distal type DVT. PE was 17 patients (17.1%), and each of patients had no symptoms. RVO was detected in 5 patients (5.0%), and two of them (2.0%) had decreased SpO2 in early 90% perioperative period. All patients did not change rehabilitation protocol and progress was good until discharge. Conclusion. We investigated consecutive 99 patients for DVT/PE after primary TKA using CE-CT and venous US. And we performed UCG to PE patients. One of the important findings to decide detail examination for asymptomatic PE was decrease in SpO2 during early perioperative period. It might be the only finding except symptom. We also have to be more aware about high incidence of asymptomatic PE and more careful to the patients after TKA to prevent complications. Introduction. Osteloysis following metal-on-UHMW polyethylene Total Hip Arthroplasty (THA) is well reported, as is lack of osteolysis following Ceramic-on-Ceramic (CoC)THA. Early ceramic failures did report some osteolysis, but in flawed implants. As 3rd and now 4th generation ceramic THAs come into mid- and long-term use, the orthopaedic community has begun to see reports of high survival rates and very low incidence of osteolysis in these bearings. Osteolysis reported after 3rd generation CoC THA often included metallosis due to neck rim impingement. In our department we have revised only 2 hips in over 6000 CoC THAs for osteolysis. Both had evidence of metallosis as well as ceramic wear. The technique used by Radiologists for identifying the nature of lesions on CT is the Hounsfield score which will identify the density of the tissue within the lucent area. It is common for radiologist to have no access to previous imaging, especially pre-operative imaging if a long time has elapsed. With such a low incidence of osteolysis in this patient group, what, then, should a surgeon do on receiving a CT report on a ceramic-on-ceramic THA which states there is osteolysis? Revision of such implants in elderly patients has a high risk of morbidity and mortality. Objectives. This retrospective review aims to determine the accuracy of CT in identifying true osteolysis in a cohort of long-term third generation ceramic-on-ceramic uncemented hip arthroplasties in our department. Methods. Pelvic CT scans were performed on the first 27 patients from a cohort of 301 patients undergoing 15 year review with 3rd generation alumina-alumina cementless THAs. The average follow-up was 15 years (15–17). The CT scans were reviewed against pre-operative and post-operative radiographs and reviewed by a second musculoskeletal specialist radiologist. Results. Eleven of the CTscans were reported to show acetabular osteolysis, two reported osteolysis or possible pre-existing cyst and one reported a definitive pre-existing cyst. After review of previous imaging including pre-operative radiographs, eleven of the thirteen patients initially reported to have osteolysis were found to have pre-existing cysts or geodes in the same size and position as the reported osteolysis, and a further patient had spot-welds with stress-shielding. One patient with evidence of true osteolysis awaits aspiration or biopsy to determine if he has evidence of ceramic wear or metallosis. Conclusions. Reports of osteolysis on CT should be interpreted with care in modern ceramic-on-ceramic THA to prevent unnecessary revision. Further imaging and investigations may be necessary to exclude other conditions such as geodes, or stress shielding which are frequently confused with osteolysis on CT scans

INTRODUCTION. Mechanical loosening of total hip replacement (THR) is primarily diagnosed using radiographs, which are diagnostically challenging and require review by experienced radiologists and orthopaedic surgeons. Automated tools that assist less-experienced clinicians and mitigate human error can reduce the risk of missed or delayed diagnosis. Thus the purposes of this study were to: 1) develop an automated tool to detect mechanical loosening of THR by training a deep convolutional neural network (CNN) using THR x-rays, and 2) visualize the CNN training process to interpret how it functions. METHODS. A retrospective study was conducted using previously collected imaging data at a single institution with IRB approval. Twenty-three patients with cementless primary THR who underwent revision surgery due to mechanical loosening (either with a loose stem and/or a loose acetabular component) had their hip x-rays evaluated immediately prior to their revision surgery (32 “loose” x-rays). A comparison group was comprised of 23 patients who underwent primary cementless THR surgery with x-rays immediately after their primary surgery (31 “not loose” x-rays). Fig. 1 shows examples of “not loose” and “loose” THR x-ray. DenseNet201-CNN was utilized by swapping the top layer with a binary classifier using 90:10 split-validation [1]. Pre-trained CNN on ImageNet [2] and not pre-trained CNN (initial zero weights) were implemented to compare the results. Saliency maps were implemented to indicate the importance of each pixel of a given x-ray on the CNN's performance [3]. RESULTS. Fig. 2 shows the saliency maps for an example x-ray and the corresponding accuracy of the CNN on the entire validation dataset at different stages of the training for both pre-trained (Fig. 2a) and not pre-trained (Fig. 2b) CNNs. Colored regions in the saliency maps, where red denotes higher relative influence than blue, indicate the most influential regions on the CNN's performance. Pre-trained CNN achieved higher accuracy (87%) on the validation set x-rays than not pre-trained CNN (62%) after 10 epochs. The pre-trained CNN's saliency map at 10 epochs identified significant influence of bone-implant interaction regions on the CNN's performance. This indicates that the CNN is ‘looking’ at the clinically relevant features in the x-rays. The saliency maps also demonstrated that the pre-trained CNN quickly learned where to ‘look’, while the not pre-trained CNN struggles. DISCUSSION. An automated tool to detect mechanical loosening of THR was developed that can potentially assist clinicians with accurate diagnosis. By visualizing the influential regions of the x-ray on the CNN performance, this study shed light into CNN learning process and demonstrated that CNN is ‘looking’ at the clinically relevant features to classify the x-rays. This visualization is crucial to build trust in the automated system by interpreting how it functions to increase the confidence in the application of artificial intelligence to the field of orthopaedics. This study also demonstrated that pre-training CNN can accelerate the learning process and achieve high accuracy even on a small dataset. The aim of this prospective study was to determine the prevalence of bone marrow oedema (BME) in asymptomatic knees of athletes and to investigate the factors associated with appearance of BME in this group. A total of 25 asymptomatic athletes who competed at an international, national or county level during their most recent sport season were recruited in this study and had MRI scan of both knees (n=50). MRI scans were reported independently by two experienced musculoskeletal radiologists. Statistical analysis included Cohen's kappa test to identify inter-observer agreement for MRI diagnosis of bone marrow oedema and multiple logistic regression model to identify the factors associated with BME on MRI scan. There was very good agreement between radiologists for diagnosis of BME (Kappa = 0.896). Seven participants (28%) were found to have BME. Six of the participants had BME in unilateral knees and one participant had BME in bilateral knees. The commonest location of BME was medial femoral condyle (62.5%) The amount of time spent in preseason training (34-38 weeks) was significantly associated with appearance of BME (P=0.048). BME seems to be common in asymptomatic athletes and one should realize that this finding might not be related to the clinical complaints of the patients. The results of the present study show that there is a training effect associated with BME. Knowing what is a ‘normal’ or ‘abnormal’ scan is important for a competitive athlete, as erroneously diagnosing BME as the underlying cause of athletes' symptoms could lead to decreased playing time or inappropriate therapy

Manual postoperative CT calculation of anteversion and inclination of the acetabular cup can be inaccurate and depends on the observer's experience. The aim of this study is to describe and present a validation of a new CT-image-based dedicate software (EGIT) for calculation of the acetabular component placement. The software principle is based on a three-dimensional reconstruction of a patient's bones from anatomical data collected postoperatively on the patient's CT scan. 15 Patient to be operated for THR were enrolled in this study. All patients were evaluated with post operative CT-scan. Measurement of Cup positioning were performed with two different methods: a manual method, performed by an expert radiologist, and a software CT image based method. Statistical analysis was performed with Intraclass Correlation Coefficent to asses interobserver and intraobserver reliability. A paired T-test was used to detect differences between manual and software methods. The Intraclass Correlation Coefficient was excellent for both the intraobserver and interobserver reliability. As expected the ICC is higher in the interobserver case. A mean cup anteversion of 14.2 (S.D. ±6.9), mean inclination of 44.2 (S.D.± 5.8) are detected with EGIT by the expert surgeon; Mean Cup anteversion of 13.6 (S.D. ± 5.11), mean inclination of 43.3 (S.D.± 5.1) are detected with manual method by expert radiologist. No statistical difference have been found (P> 0.05). The EGIT software seems to be an easy, accurate and reproducible method to calculate acetabular cup positioning using standard post-operative CT scan in THA

Introduction. Literature in respect to the MRI appearances of surgically confirmed spring ligament pathology is sparse. The authors conducted a retrospective review of MRI examinations comprising 13 patients with surgically proven spring ligament abnormality. Methods. Records for operations performed for planovalgus foot deformity with operation notes confirming presence of spring ligament abnormality were obtained for patients treated 2010–11. Of 32 procedures 13 patients (3 male, 10 female) mean age 48.5 (range, 21–86 years) underwent preoperative MRI scanning using a standard musculoskeletal protocol on a T1.5 unit. Scans were retrospectively reviewed by one of the senior authors and consultant musculoskeletal radiologist for pathological findings. Results. The superio-medial portion of the ligament was abnormal in all 13 scans, most common pathological findings were: thickening of the ligament proximally (>5mm) with distal thinning (< 2mm). Global thickening was confirmed on 1 scan as abnormal with a final abnormality on 1 scan of high signal change at the navicular insertion representing a linear tear. The medio-plantar portion demonstrated less consistent abnormal findings: 5 scans demonstrated high midsubstance or navicular insertion signal change. Abnormal widening of 7–9.5mm was noted in 4 scans, thinning and loss of striations was reported in 2 scans. For the final 2 scans no abnormality was detected. Conclusion. The most consistent abnormal finding representing spring ligament pathology in this series was thickening/bunching of the superior-medial portion of the ligament >5mm and thinning/absence of the ligaments insertion into the navicular distally (one or more of these features were present in 85% of confirmed ruptures). Our series concurs with the current literature in that surgically significant pathology of the spring ligament complex is best demonstrated in the superior-medial portion on proton density fat-saturated axial cuts, surgeons and radiologists should focus on this sequence for the detection of surgically relevant spring ligament abnormality

Aims

The standard of wide tumour-like resection for chronic osteomyelitis (COM) has been challenged recently by adequate debridement. This paper reviews the evolution of surgical debridement for long bone COM, and presents the outcome of adequate debridement in a tertiary bone infection unit.

Methods

We analyzed the retrospective record review from 2014 to 2020 of patients with long bone COM. All were managed by multidisciplinary infection team (MDT) protocol. Adequate debridement was employed for all cases, and no case of wide resection was included.
