Introduction and Objective. Difficult primary total knee arthroplasty (TKA) and revision TKA are high demanding procedures. Joint exposure is the first issue to face off, in order to achieve a good result. Aim of this study is to evaluate the clinical and radiological outcomes of a series of patients, who underwent TKA and revision TKA, where tibial tubercle osteotomy (TTO) was performed. Materials and Methods. We retrospectively reviewed a cohort of 79 consecutives TKAs where TTO was performed, from our Institution registry. Patients were assessed clinically and radiographically at their last follow-up (mean, 7.4 ± 3.7 years). Clinical evaluation included the Knee Society Score (KSS), the pain visual analogue scale (VAS), and range of motion. Radiological assessment included the evaluation of radiolucent lines, osteolysis, cortical bone hypertrophy, time of bone healing of the TTO fragment, and the hardware complication. Results. KSS raised from 40.7 ± 3.1 to 75 ± 4.3 (p < 0.0001). Knee flexion increased from 78.7 ± 9.9° to 95.0 ± 9.5° (p < 0.0001), and VAS improved from 7.9 ± 0.9 to 3.8 ± 1 (p < 0.0001). No signs of loosening or evolutive radiolucency lineswere found. Osteolytic areas around the stem were detected. No significant association was found between the implant design and the outcomes, while aseptic loosening showed significantly better results. Complications were: 4 painful hardware, 3 late periprosthetic infections, 1 extension lag of 5°, and 3 flexion lag. Conclusions. Our experience suggests the use of TTO to improve the surgical approach in difficult primary TKA or revision TKA. A strict surgical technique leads to good results with low risk of complications

Outcomes for guided motion primary total knee arthroplasty (TKA) in obese patients are unknown. 1,684 consecutive patients underwent 2,059 primary TKAs with a second-generation guided motion implant between 2011–2017 at three European and seven US sites. Of 2,003 (97.3%) TKAs in 1,644 patients with BMI data: average age 64.5 years; 58.4% females; average BMI 32.5 kg/m2; 13.4% had BMI ≥ 40 kg/m2. Subjects with BMI ≥ 40 kg/m2 had longest length of hospital stay (LOS) at European sites; LOS similar at US sites. Subjects with BMI ≥ 40 kg/m2 (P=0.0349) had longest surgery duration. BMI ≥ 40 kg/m2 had more re-hospitalizations or post-TKA reoperations than BMI < 40 kg/m2 (12.7% and 9.2% at five-year post-TKA, P<0.0495). Surgery duration and long-term complication rates are higher in patients with BMI ≥ 40 kg/m2, but device revision risk is not elevated

Abstract. Objectives. Patella resurfacing in primary total knee arthroplasty (TKA) remains a contentious issue. Australian rates of patellar resurfacing are 66.6%, significantly higher compared to UK rates of 8–15% and Swedish rates of 2%. Resurfacing has gained popularity in Australia since registry data has shown decreased revision rates with no increase in patellar component related complications. We present for discussion an analysis of 113,694 total knee arthroplasties using commonly implanted prostheses in the UK. Methods. We included all TKA's since the Australian register's conception on 01/09/1999 for a primary diagnosis of osteoarthritis involving the use of either the Triathlon or Duracon implant with and without patellar components. The primary outcome of the study was time to revision for Triathlon's resurfaced and non-resurfaced prosthesis compared to the Duracon's equivalent data. We also analysed the reasons for revision between the 4 groups, type of revision and complication rates. We then compared minimally stabilised and posterior stabilised prostheses. Results. The cumulative revision rate for Triathlon prostheses with resurfacing after 12 years was 3.2% (95% CI, 2.9% to 3.6%) compared to 5.6% (95% CI, 5.0% to 6.2%) without resurfacing. Duracon's equivalent data was 6.3% with resurfacing and 5.9% without resurfacing. Triathlon prosthesis with resurfacing have much lower rates of revision due to loosening, patellofemoral pain, patellar erosion compared to unresurfaced Triathlon prostheses. Conclusion. Triathlon with re-surfacing has lower revision rates regardless of age or BMI. Previous concerns regarding patellofemoral loosening, tibial wear, maltracking relate to Duracon only, indicating the importance of implant specific studies. Declaration of Interest. Abstract. Objective. In this systematic review we aim to analyse the economical impact of using Negative Pressure Wound Therapy (NPWT) in primary total knee arthroplasty (TKA). Methods. Four medical electronic databases were searched. Eligible studies included those investigating the costs of NPWT in primary TKA. Exclusion criteria included studies investigating cost of NPWT not related to primary TKA. We also excluded studies with poor scientific methodology. We retrieved and analysed data on dressing costs and hospital length of stay (LOS). Results. Three studies (359 patients) reported on dressing and associated health care costs, and two further studies (330 patients) reported on hospital LOS in primary TKA. The cost of NPWT ranged between £125 and £196; with an average cost of £155, compared to £23 for the regular surgical dressing. The hospital LOS in NWPT patients ranged from 1.9 – 3.8 days, while LOS in patients managed with regular surgical dressing ranged between 2.3 – 4.7 days. The hospital LOS accounted for delayed discharge due to wound complications. Any extended LOS secondary to medical comorbidities or for other reasons were not included here. Conclusion. Our pooled analysis found a decrease in hospital LOS from wound related problems when using NWPT instead of regular dressings after accounting for other variables responsible for LOS. If the mean cost of overnight inpatient hospital stay for elective TKA is estimated as £275, the range of overnight admission cost for one TKA patient would be £522 - £1045 when NPWT dressing is used, and £632.5 - £1292 when using regular dressings. The cost savings from reduced LOS amounts to £110 - £247 per patient when NPWT is used. We hypothesize that in primary TKA patients with high risk of wound related problems that may delay discharge from hospital, there may be an overall cost saving when using NWPT dressings. Introduction. As the demand for primary total knee arthroplasty (TKA) has been on the rise, so will be the demand for revision knee surgery. Nevertheless, our knowledge on the modes of failure and factors associated with failure of knee revision surgery is considerably lower to that known for primary TKA. To date, this has been mostly based on case series within the literature. Therefore, the aim of this study was to evaluate the survivorship of revision TKA and determine the reasons of failure. Methods. A retrospective study was conducted with prior approval of the institutional audit department. This involved evaluation of existing clinical records and radiographs of patients who underwent revision knee surgery at our institution between 2003 and 2015. Re-revision was identified as the third or further procedure on the knee in which at least one prosthetic component was inserted or changed. Results. 95 patients were identified who had re-revision knee replacement. Of these, there were 46 men (48%) and 49 women (52%) with an average age of 65 yrs. Infection was the main cause of failure (35.8%) followed by aseptic loosening (27.4%) and extensor mechanism problems leading to reduced range of motion (7.4%). Other causes included MCL Laxity (3.2%), oversized implants (3.2%) and fracture of the cement mantle (1.1%). The mean survivorship of revision knee replacements in our cohort was 31 months (Range between 1 – 119 months). Conclusion. The survival of knee re-revision implants is considerably different from primary TKA implants. The results of our study provide an improved understanding of the modes of failure of re-revision knee replacements, enabling orthopaedic surgeons the opportunity to better understand the current problems associated with revision knee surgery and the potential to improve our outcomes by tackling these issues

Introduction. With the development of 3D printing technology, there are many different types of PSI in the world. The accuracy of patient specific instrumentation (PSI) in primary total knee arthroplasty (TKA) is dependent on appropriate placement of the cutting blocks. However, previous reports on one type of PSI measured the difference between postoperative prosthetic alignment and postoperative mechanical axis and thus these reports did not evaluate intraoperative comparison of PSIs between two different designs. The purpose of this study was to evaluate the intraoperative accuracy of two different designed PSIs (My knee, Medacta International, Castel San Pietro, Switzerland) with two examiners using CT free navigation system (Stryker, Mahwar, NJ, USA) in regards to sagittal and coronal alignment. Methods. We enrolled 78knees (66 patients) with a primary cemented TKA using two different designed CT-based PSIs (My knee, Medacta International, Castel San Pietro, Switzerland). All operations were performed by two senior surgeons who have experience with greater than 500 TKAs and greater than 200 navigated TKAs. Two examiners were same two surgeons. The study period was between June 2015 and November 2016. The local ethics' committee approved the study prior to its initiation, and informed consent was obtained from all patients. After placement of the PSI on the femur and tibia, the position of the PSI was evaluated by s intraoperative navigation. Two examiners placed two different types (STD(standard) and MIS(minimum invasive surgery)) of PSI on same joint. As required by the PSI, only soft- tissue was removed and osteophytes were left in place. Femoral MIS PSI was required partial remove of lateral cartilage. For the femur, the coronal position in relation to the mechanical axis were documented. For the tibia, the coronal alignment and the tibial slope were documented. Of note, intraoperative modifications to the PSI were not made based upon the results of the navigation. Rather, the findings of the intraoperative navigation were simply documented. Results. The mean age of the cohort was 72.9±7.5years (range, 55–85years). The study included 11men and 55women, with a mean height of 151±8.2cm (range, 135–175cm), mean weight of 59.4±4.3kg (range, 42–82kg), and a mean of Body Mass Index of 25.9±3.6 (range, 17.2–36.4). HKA angle (supine position) measured by CT was 170.8 ±4.4 degree(range, 162.5–182degree). Diagnosis was osteoarthritis in all patient. There was no statistically significant difference in PSI position alignment for femoral flexion, tibial coronal angle, tibial slope between the two groups with two examiners. However, the intraoperative coronal position using the femoral STD PSI significantly deviated from using femoral MIS PSI from both examiners. (PSI vs. MIS, examiner1 p = 0.02, examiner2 p=0.04)

Introduction. Following primary total knee arthroplasty (TKA), patients experience pain relief and report improved physical function and activity. However, there is paucity of evidence that patients are truly more active in daily life after TKA. The aims of this study were: 1) to prospectively measure physical activity with a wearable motion sensor before and after TKA; 2) to compare patient-reported levels of physical activity with objectively assessed levels of physical activity before and after TKA; 3) to investigate whether differences in physical activity after TKA are related to levels of physical function. Methods. 22 patients (age=66.6 ±9.3yrs; m/f= 12/11; BMI= 30.6 ±6.1) undergoing primary TKA (Vanguard, ZimmerBiomet), were measured preoperatively and 1–3 years postoperatively. Patient-reported outcome measures (PROMs) included KOOS-PS and SQUASH for assessment of perceived physical function and activity resp. Physical activity was assessed during 4 consecutive days in patients” home environments while wearing an accelerometer-based activity monitor (AM) at the thigh. All data were analysed using semi-automated algorithms in Matlab. AM-derived parameters included walking time (s), sitting time (s) standing time (s), sit-to-stand transfers, step count, walking bouts and walking cadence (steps/min). Objective physical function was assessed by motion analysis of gait, sit-to-stand (STS) transfers and block step-up (BS) transfers using a single inertial measurement unit (IMU) worn at the pelvis. IMU-based motion analysis was only performed postoperatively. Statistical comparisons were performed with SPSS and a per-protocol analysis was applied to present the results at follow-up. Results. Data were available for 17 of 22 patients at follow-up. PROMs demonstrated significant improvement of perceived physical function (KOOS-PS=68±21 vs. 34±26; p<0.001) and physical activity (SQUASH=2584 ±1945 vs. 3038 ±2228; p<0.001) following TKA. AM-based parameters of physical activity demonstrated no significant differences between pre- and postoperative quantitative outcomes. Only the qualitative outcome of walking cadence significantly changed after TKA (81.41 ±10.86 (steps/min) vs. 94.24 ±7.20 resp.; p<0.001). There were moderate correlations between self-reported and objectively assessed levels of physical activity after TKA (Pearson”s r=0.36–0.43; p<0.05). Outcomes of physical activity after TKA were moderately correlated to IMU-based functional outcome measures (Pearson”s r = 0.31 – 0.48; p<0.05). Conclusion. 1–3 years after TKA, patients demonstrate improved function. However, the self-perceived higher activity level (+18%) after TKA is not supported by any objective data obtained by wearable motion sensors such as steps, transfers or time-on-feet. This may have implications for general health and requires further investigation into patient communication, expectation management or motivational intervention

The number of primary Total Knee Arthroplasty (TKA) and primary Total Hip Arthroplasty (THA) procedures carried out in England and Wales is increasing annually. The British Orthopaedic Association guidelines for follow up currently differ for patients with TKA and THA. In THA the BOA recommends that Orthopaedic Data Evaluation Panel (ODEP) 10A rated implants should be followed up in the first year, once at seven years and three yearly thereafter. The BOA guidelines for TKA minimum requirement is radiographs at 5 years and each five years thereafter. Few studies have investigated if early follow up affects patient management following total hip and knee arthroplasty. We carried out a retrospective review of all revision procedures carried out in our institution between April 2010 to April 2013. The medical notes and radiographs for each patient were examined to determine the operative indications and patients symptoms. 92 knee revisions and 143 hip revisions were identified. Additionally we retrospectively reviewed the outcome of 300 one year routine arthroplasty follow up appointments. The mean time of hip revision was 8.5years (range 0 to 27years) and 5.6years (range 0 to 20years) for knee revisions. The commonest cause for revision was aseptic loosening associated with pain in 49 (53%) of knee revision patients and 89 (63%) of hip revisions. Infection accounted for 26 (28%) knee revisions and 16 (12%) hip revisions. Only 1% of hip and knee revisions was carried out in asymptomatic patients with aseptic loosening. We did not identify any cases were a patients management was altered at the routine arthroplasty review clinic and none were referred on for further surgical treatment. The findings of our study suggest there is no evidence for a routine one year arthroplasty review and revisions were carried out in asymptomatic patients in 1% of patients

The Columbus is a relatively new implant with no published medium or long term follow-up. Its extensive use within our department led to this study to review the five-year clinical outcomesof patients who had a navigated Columbus primary total knee arthroplasty (TKA) implanted between March 2005 and December 2006. Case notes, departmental and hospital databases and PACS were used to identify patients and collect routine five-year review data. Information Services Division was approached for all cases of re-admission and associated complications anywhere in Scotland. 219 (90 male, 116 left) patients were identified. Mean age was 69 years (48–89) and mean BMI 32.2 (SD 5.8). Of the 219 patients operated on, twenty-one had a complication; ten still had intermittent mild to moderate pain, three had wound problems, one had a washout, one had DVT/PE within ninety days and one was diagnosed with patellar clunk syndrome. The remaining five patients had revision TKA (revision rate 2.3%), four for infection and only one due to aseptic loosening (revision rate excluding infection 0.5%). 115 patients returned to clinic at five years. Of these 96.4% were satisfied with their operation. The mean Oxford knee score had reduced from 42.5 (SD 8.2) pre-operatively to 23.6 (SD 9.2). Mean fixed flexion was 1° (SD 2.8°, range 0° to 15°) with four patients having a fixed flexion of 6° or more. Mean maximum flexion was 100° (SD 10.2°, range 60° to 120°) with two patients having flexion less than 80°. X-ray analysis showed that fourteen patients had a radiolucent line. Nine of these were not present at one year, six being at the tibial component. These results are satisfactory. The revision rate is similar to that cited by the National Joint Registry report 2011 (2.5%). Furthermore, the revision rate excluding infection is very low

Summary. Computer assisted surgery (CAS) during total knee arthroplasty (TKA) is known to improve prosthetic alignment in coronal and sagittal plane. In this systematic review, no evidence is found that CAS also improves axial component orientation when used during TKA. Introduction. Primary total knee arthroplasty (TKA) is a safe and cost-effective treatment for end-stage knee osteoarthritis. Correct prosthesis alignment is essential, since malpositioning of the prosthesis leads to worse functional outcome and increased wear, which compromises survival of the prosthesis. Computer assisted surgery (CAS) has been developed to enhance prosthesis alignment during TKA. CAS significantly improves postoperative coronal and sagittal alignment compared to conventional TKA. However, the influence of CAS on rotational alignment is a matter of debate. Therefore purpose of this review is to assess published evidence on the influence of CAS during TKA on postoperative rotational alignment. Patients and Methods. This review was performed according to the PRISMA Statement. An electronic literature search was performed in Pubmed, Medline and Embase on studies published between 1991 and April 2013. Studies were included when rotational alignment following imageless CAS-TKA was compared to rotational alignment following conventional TKA. At least one of the following outcome measures had to be assessed: 1) rotational alignment of the femoral component, 2) rotational alignment of the tibial component, 3) tibiofemoral mismatch, 4) the amount of rotational outliers of the femoral component, 5) the amount of rotational outliers of the tibial component. Study selection was performed in two stages and data extraction and methodological quality assessment was conducted independently by two reviewers. Standardized mean difference (SMD) with 95% confidence interval (95% CI) was calculated for continuous variables. The SMDs were interpreted according to Cohen: an SMD of 0.2–0.4 was considered a small effect; 0.5–0.7 was considered moderate; and ≥ 0.8 was considered a large effect. For the comparison of the amount of outliers for femoral and tibial component rotation, the Odds ratio (OR) and 95% CI was calculated. The OR represents the odds of outliers occurring in the CAS group compared with the conventional group. An OR of < 1 favors the CAS group. The OR is considered statistically significant when the 95% CI does not include the value of 1. Results. Seventeen studies met the inclusion criteria. One study was considered of high, 15 studies of medium and one study of low methodological quality. SMD for rotation of the femoral component was −0.07 (−0.19–0.04). For rotation of the tibial component, the SMD was 0.11 (−0.01–0.24). Regarding tibiofemoral mismatch, the SMD was −0.27 (−0.57–0.02). For femoral outliers, the OR was 1.05 (0.78–1.43) and for tibial outliers the OR was 1.12 (0.86–1.47). Discussion / Conclusion. Results of this review show no evidence that CAS-TKA leads to better rotational alignment of the femoral or tibial component or tibiofemoral mismatch. Also no evidence was found that CAS results in a decrease of the amount of outliers regarding femoral or tibial component orientation. However, these conclusions have to be interpreted with caution. The number of included studies was low and strong heterogeneity existed between the studies. Of the 17 included studies, only one study was considered of high methodological quality. Objectives

The objective of this study was to determine if combining variations in mixing technique of antibiotic-impregnated polymethylmethacrylate (PMMA) cement with low frequency ultrasound (LFUS) improves antibiotic elution during the initial high phase (Phase I) and subsequent low phase (Phase II) while not diminishing mechanical strength.

Methods

Three batches of vancomycin-loaded PMMA were prepared with different mixing techniques: a standard technique; a delayed technique; and a control without antibiotic. Daily elution samples were analysed using flow injection analysis (FIA). Beginning in Phase II, samples from each mix group were selected randomly to undergo either five, 15, 45, or 0 minutes of LFUS treatment. Elution amounts between LFUS treatments were analysed. Following Phase II, compression testing was done to quantify strength. A-priorit-tests and univariate ANOVAs were used to compare elution and mechanical test results between the two mix groups and the control group.
