Dabigatran etexilate (Pradaxa®) is an oral direct thrombin inhibitor that was recently approved in Europe and Canada for the prevention of venous thromboembolism (VTE) in patients undergoing elective total knee replacement or total hip replacement surgery. Two pivotal clinical trials, RE-MODEL (
Osteoporosis is common and the health and financial
cost of fragility fractures is considerable. The burden of cardiovascular
disease has been reduced dramatically by identifying and targeting
those most at risk. A similar approach is potentially possible in
the context of fragility fractures. The World Health Organization
created and endorsed the use of FRAX, a fracture risk assessment
tool, which uses selected risk factors to calculate a quantitative,
patient-specific, ten-year risk of sustaining a fragility fracture.
Treatment can thus be based on this as well as on measured bone
mineral density. It may also be used to determine at-risk individuals,
who should undergo bone densitometry. FRAX has been incorporated
into the national osteoporosis guidelines of countries in the Americas,
Europe, the Far East and Australasia. The United Kingdom National
Institute for Health and Clinical Excellence also advocates its
use in their guidance on the assessment of the risk of fragility
fracture, and it may become an important tool to combat the health
challenges posed by fragility fractures.
Rivaroxaban, an oral, direct FXa inhibitor has shown in large phase III trials to be both superior to enoxaparin a low molecular weight heparin for VTE prophylaxis in patients undergoing MOS, and to also have a good safety profile. RECORD, a pivotal clinical trial program investigating rivaroxaban for the prevention of VTE after THR and TKR surgery, consists of four multinational, randomized, double-blind, double-dummy phase III studies (RECORD1,2,3 and 4) comparing rivaroxaban 10 mg once-daily with enoxaparin 40 mg once-daily or 30 mg twice-daily. The RECORD program has consistently shown superiority of rivaroxaban to enoxaparin at preventing VTE after major orthopaedic surgery. Results from the RECORD 2 study confirmed the benefit of extended thromboprophylaxis after THR. Rivaroxaban was more effective than enoxaparin at reducing the incidence of VTE and all course mortality in patients undergoing THR, with a relative risk reduction (RRR) of 70% in total VTE (RECORD 1). In the TKR populations, rivaroxaban was superior to both once-daily (RECORD 3) and twice-daily (RECORD 4) enoxaparin, with a RRR of 49% and 31.4%, respectively. It also significantly reduced the incidence of symptomatic VTE in TKR patients (RECORD 3). Rivaroxaban groups had low and similar bleeding rates to enoxaparin across the RECORD program. Thus, with its superior efficacy and a good safety profile, oral, once-daily fixed dosing with rivaroxaban could transform the future of VTE prevention after major orthopaedic surgery and improve the quality and reliability of patients care.
Periprosthetic joint infection (PJI) occurs in 0.2-2% of primary hip and knee arthroplasty and is a leading cause of revision surgery, impaired function, and increased morbidity and mortality. Topical, intrawound vancomycin administration allows for high local drug concentrations at the surgical site and has demonstrated good results in prevention of surgical site infection after spinal surgery. It is a promising treatment to prevent infection following hip and knee arthroplasty. Prior studies have been limited by small sample sizes and the low incidence of PJI. Objectives. Bisphosphonates are widely used as first-line treatment for primary and secondary prevention of fragility fractures. Whilst they have proved effective in this role, there is growing concern over their long-term use, with much evidence linking bisphosphonate-related suppression of bone remodelling to an increased risk of atypical subtrochanteric fractures of the femur (AFFs). The objective of this article is to review this evidence, while presenting the current available strategies for the management of AFFs. Methods. We present an evaluation of current literature relating to the pathogenesis and treatment of AFFs in the context of bisphosphonate use. Results. Six broad themes relating to the pathogenesis and management of bisphosphonate-related AFFs are presented. The key themes in fracture pathogenesis are: bone microdamage accumulation; altered bone mineralisation and altered collagen formation. Mendelian randomization (MR) is considered to overcome the bias of observational studies, but there is no current meta-analysis of MR studies on rheumatoid arthritis (RA). The purpose of this study was to summarize the relationship between potential pathogenic factors and RA risk based on existing MR studies. PubMed, Web of Science, and Embase were searched for MR studies on influencing factors in relation to RA up to October 2022. Meta-analyses of MR studies assessing correlations between various potential pathogenic factors and RA were conducted. Random-effect and fixed-effect models were used to synthesize the odds ratios of various pathogenic factors and RA. The quality of the study was assessed using the Strengthening the Reporting of Observational Studies in Epidemiology using Mendelian Randomization (STROBE-MR) guidelines.Aims

The aim was to perform a cost-utility analysis of total joint replacement in the current environment. Arthritis is a disabling condition that leads to long-term deterioration in quality of life. Total joint replacement, despite being one of the greatest advances in medicine in the modern era, has come under recent scrutiny. The National Health Service (NHS) has competing demands and resource allocation is challenging in times of economic restraint. Patients undergoing total hip (n=348) and knee arthroplasty (n=323), from January to July 2010, were entered into a prospective arthroplasty database. A health utility score was derived from the Euroqol (EQ-5D) score preoperatively, and at one year, and was combined with individual life expectancy to derive the Quality-Adujusted-Life-Years (QALYs) gained. Predicted need for revision surgery was Incorporated in the model. The 2011–12 Scottish Tariff was used. Two-way analysis of variance was used to compare QALYs gained between procedures, while controlling for baseline differences. The number of QALYs gained was higher in THR versus TKR (6.53 vs 4.04 years, p<0.001). The cost per QALY for THR was £1371 (95% CI £1194 to £1614) compared with £2101 (£1762 to £2620) for TKR. Predictors of an increase in QALYs gained were poorer health prior to surgery (p<0.001) and younger age (p<0.001). General health (EQ-5D VAS) showed greater improvement in THR versus TKR (p<0.001). This study provides up to date cost-effectiveness data for total joint replacement. THR and TKR are both extremely clinically and cost-effective interventions, with costs that compare favourably with other medical interventions (e.g. laparoscopic hernia repair vs open: £55,548 per QALY; CHD
Background. The conflict in Afghanistan has been epitomised by the emergence of the Improvised Explosive Device (IEDs). Improvements in protection and medical treatments have resulted in increasing numbers of casualties surviving with complex lower extremity injuries. To date, there has been no analysis of foot and ankle blast injuries as a result of IEDs. Therefore the aims of this study are to report the pattern of injury and determine which factors were associated with a poor clinical outcome. Methods. Using a prospective trauma registry, UK Service Personnel who sustained lower leg injuries following an under-vehicle explosion between Jan 2006 and Dec 2008 were identified. Patient demographics, injury severity, the nature of lower limb injury and clinical management was recorded. Clinical endpoints were determined by

Results. 63 UK Service Personnel (89 injured limbs) were identified with lower leg injuries from explosion. 50% of casualties sustained multi-segmental injuries to the foot and ankle complex. 26(29%) limbs required amputation, with six amputated for chronic pain 18 months following injury. Regression analysis revealed that hindfoot injuries, open fractures and vascular injuries were independent predictors of amputation. Of the 69 limbs initially salvaged, the overall infection rate was 42%, osteomyelitis 11.6% and non-union rates was 21.7%. Symptomatic traumatic osteoarthritis was noted in 33.3% salvaged limbs. At final follow-up, 66(74%) of injured limbs had persisting symptoms related to their injury, with only 9(14%) fit to return to their pre-injury duties. Conclusions. This study demonstrates that foot and ankle injuries from IEDs are frequently associated with a high amputation rate and poor clinical outcome. Although, not life-threatening, they remain a source of long-term morbidity in an active population.

Calcium and vitamin D are both of key importance for bone health, and their effects on bone appear to begin even in utero and continue throughout life. The dietary requirements for both calcium and vitamin D are different at different stages of the lifespan. Importantly, in Australia the bulk of vitamin D comes from manufacture of vitamin D in the skin from ultraviolet light exposure i.e. from sun exposure, as the amount of vitamin D in foods is low. Vitamin D deficiency is common at all stages of life and some groups are at particularly high risk. Adequate calcium intake and maintaining adequate vitamin D levels are important in childhood for maximising peak bone mass, but the effect of calcium supplementation on bone mineral density is small. The role of vitamin D supplementation in childhood outside of treating rickets is unclear, though there is potential for a clinically significant effect. Anti-vehicle mines (AV) and Improvised Explosive Devices (IEDs) remain the most prevalent threat to Coalition troops operating in Iraq and Afghanistan. Detonation of these devices results in rapid deflection of the vehicle floor resulting in severe injuries to calcaneus. Anecdotally referred to as a ‘deck-slap’ injury, there have been no studies evaluating the pattern of injury or the effect of these potentially devastating injuries since World War II. Therefore the aim of this study is to determine the pattern of injury, medical management and functional outcome of UK Service Personnel sustaining calcaneal injuries from under-vehicle explosions. From Jan 2006 – Dec 2008, utilising a prospectively collected trauma registry (Joint Theatre Trauma Registry, JTTR), the records of all UK Service Personnel sustaining a fractured calcaneus from a vehicle explosion were identified for in depth review. For each patient, demographic data, New Injury Severity Score (NISS), and associated injuries were recorded. In addition, the pattern of calcaneal fracture, the method of stabilisation, local complications and need for amputation was noted. Functional recovery was related to the ability of the casualty to return to military duties. Forty calcaneal fractures (30 patients) were identified in this study. Mean follow-up was 33.2 months. The median NISS was 17, with the lower extremity the most severely injured body region in 90% of cases. Nine (30%) had an associated spinal injury. The overall amputation rate was 45% (18/40); 11 limbs (28%) were amputated primarily, with a further 3 amputated on return to the UK. Four (10%) casualties required a delayed amputation for chronic pain (mean 19.5 months). Of the 29 calcaneal fractures salvaged at the field hospital, wound infection developed in 11 (38%). At final follow-up, only 2 (6%) were able to return to full military duty with 23 (76%) only fit for sedentary work or unfit any military duty. Calcaneal injuries following under-vehicle explosions are commonly associated with significant polytrauma, of which the lower limb injury is the most severe. Spinal injuries were frequently associated with this injury pattern and it is recommended that radiological evaluation of the spine be performed on all patients presenting with calcaneal injuries from this injury mechanism. The severity of the hindfoot injury witnessed is reflected by the high infection rate and amputation rate seen in this cohort of patients. Given the high physical demands of a young, active military population, only a small proportion of casualties were able to return to pre-injury duties. The number of people suffering from pain or limitation of daily activities as a result of conditions related to the musculoskeletal system is increasing in Europe and worldwide. It is therefore essential to develop strategies to prevent both the occurrence of these conditions and the impact of these conditions. Most musculoskeletal conditions occur in the elderly and as the elderly population will reach above 20% within the next 20 years this will further augment the problem within Europe, particularly as these conditions also increase with advancing age. It must be recognized that at most levels within the health care systems or within society, the impact of these conditions today and for the future is underestimated, both regarding number and regarding consequences; disability, handicap, social implications and costs. The major challenge is, however, not to make recommendations for preventive strategies but to implement them in order to secure a change, leading to improved care and improved quality of life for patient, regardless of age. The first step in order to make a change is to define the size of the problem – the burden of musculoskeletal conditions. With respect to the incidence and prevalence the size of the problem is relatively well known. The size of the problem when it comes to the impact on the individual is less well known, as is the burden in terms of economic and societal costs. For many conditions the risk factors are identified and common to many conditions. The second step involves evidence – evidence for the effectiveness of the interventions available today. When evaluating evidence it is useful to define population in terms of the normal or healthy population, those at risk, those at early or moderate stage and those at late stage of the condition since this will translate into preventive strategies appropriate for each level. The final step is implementation and this is the major challenge at all levels, from policies trying to influence the entire population to adopt a healthier life style, to change the management of the individual patient in the doctor’s office. Based on knowledge of size and evidence, it is possible to transform recommendations into an actual action plan at national, regional and local levels. The principles for successful implementations are to a large extent similar for all levels. The tools for success need to be identified and they may include financial or economic mechanisms, regulatory mechanisms, and educational or organisational mechanisms. It is particularly important to identify barriers and facilitators that will influence the outcome of the proposed strategy. Musculoskeletal conditions have many risk factors and interventions in common with other conditions that affect public health, such as diabetes and cardiovascular conditions. In
Study design: A systematic review of the literature to inform the development of occupational health guidelines for the management of low back pain at work. Objectives: To evaluate the evidence from occupational health settings or concerning occupational outcomes. Summary of background: Clinical guidelines for the management of low back pain (LBP) provide only limited guidance on the occupational aspects. Thus the Faculty of Occupational Medicine requested this review in order that a multi-disciplinary working group could develop the first evidence-based UK guidelines for management of LBP at work. Methods: A systematic literature search was followed by rating of the strength of the evidence plus a narrative review, by agreement between two experienced and independently-minded reviewers. There was no attempt at blinded double review or quality scoring. The final version followed peer-review by four international experts. Results: More than 2000 titles were considered. 34 systematic reviews, 28 narrative reviews, 52 additional scientific studies, 22 less rigorous scientific studies and 17 previous guidelines were identified and included. The evidence statements (rated for strength) were presented under headings that reflect a logical sequence of occupational health situations (Background, Pre-placement assessment, Prevention, Assessment of the worker presenting with back pain, Management principles for the worker presenting with back pain, Management of the worker having difficulty returning to normal occupational duties at 4–12 weeks). Some important areas were given additional narrative evidence-linked discussion (High risk patients/physically demanding jobs, Return to work with back pain, Rehabilitation programmes). Thirty six evidence-linked statements were developed to inform the guidelines group. The strongest evidence suggests that: generally the physical demands at work have only a modest influence on the incidence of LBP or permanent spinal damage; a history of LBP is not a reason to deny employment; preventive strategies based on the injury model do not reduce LBP or work loss; individual and work-related psychosocial factors play an important role in persisting symptoms and work loss; the management approach should be ‘active’ (including early work return); the combination of clinical, rehabilitation and organisational interventions designed to assist work return is more effective than single elements. However, further research is needed to identify the optimal roles of all stakeholders (clinicians, employers and workers) in case management. Conclusions: This review consolidates the emerging focus on active management of LBP at work, and indicates that approaches addressing obstacles to recovery will provide greater benefits than attempts at
Rehabilitation as a concept, and the practice of rehabilitation, have changed remarkably during the preceding years. Modern rehabilitation is multidisciplinary and multi-professional. The development and research of rehabilitation has also grown international. The contents of modern rehabilitation include medical, social and psychological aspects, and in vocational rehabilitation, the working conditions and organisational questions are dealt with as well (Jager 1999). Traditional methods in vocational rehabilitation and guidance include medical and psychological assessment, and work clinics assessment (which includes practical work assessment in the work place). Presently, the methods, have advanced and rehabilitation is not seen merely as a specific method for handicapped people. Various rehabilitation measures are flexibly applied, and rehabilitation includes elements of adult education, training of working skills, and ideas of on-the-job training and career advancement. Degenerative disc disease (DDD) and osteoarthritis (OA) are relatively frequent causes of disability amongst the elderly; they constitute serious socioeconomic costs and significantly impair quality of life. Previous studies to date have found that aggrecan variable number of tandem repeats (VNTR) contributes both to DDD and OA. However, current data are not consistent across studies. The purpose of this study was to evaluate systematically the relationship between aggrecan VNTR, and DDD and/or OA. This study used a highly sensitive search strategy to identify all published studies related to the relationship between aggrecan VNTR and both DDD and OA in multiple databases from January 1996 to December 2016. All identified studies were systematically evaluated using specific inclusion and exclusion criteria. Cochrane methodology was also applied to the results of this study.Objectives
Methods
The ageing population and an increase in both
the incidence and prevalence of cancer pose a healthcare challenge, some
of which is borne by the orthopaedic community in the form of osteoporotic
fractures and metastatic bone disease. In recent years there has
been an increasing understanding of the pathways involved in bone
metabolism relevant to osteoporosis and metastases in bone. Newer
therapies may aid the management of these problems. One group of
drugs, the antibody mediated anti-resorptive therapies (AMARTs)
use antibodies to block bone resorption pathways. This review seeks
to present a synopsis of the guidelines, pharmacology and potential pathophysiology
of AMARTs and other new anti-resorptive drugs. We evaluate the literature relating to AMARTs and new anti-resorptives
with special attention on those approved for use in clinical practice. Denosumab, a monoclonal antibody against Receptor Activator for
Nuclear Factor Kappa-B Ligand. It is the first AMART approved by
the National Institute for Health and Clinical Excellence and the
US Food and Drug Administration. Other novel anti-resorptives awaiting
approval for clinical use include Odanacatib. Denosumab is indicated for the treatment of osteoporosis and
prevention of the complications of bone metastases. Recent evidence
suggests, however, that denosumab may have an adverse event profile
similar to bisphosphonates, including atypical femoral fractures.
It is, therefore, essential that orthopaedic surgeons are conversant
with these medications and their safe usage. Take home message: Denosumab has important orthopaedic indications
and has been shown to significantly reduce patient morbidity in
osteoporosis and metastatic bone disease. Cite this article:
Neurogenic heterotopic ossification (NHO) is
a disorder of aberrant bone formation affecting one in five patients sustaining
a spinal cord injury or traumatic brain injury. Ectopic bone forms
around joints in characteristic patterns, causing pain and limiting
movement especially around the hip and elbow. Clinical sequelae
of neurogenic heterotopic ossification include urinary tract infection,
pressure injuries, pneumonia and poor hygiene, making early diagnosis
and treatment clinically compelling. However, diagnosis remains
difficult with more investigation needed. Our pathophysiological
understanding stems from mechanisms of basic bone formation enhanced
by evidence of systemic influences from circulating humor factors
and perhaps neurological ones. This increasing understanding guides
our implementation of current prophylaxis and treatment including
the use of non-steroidal anti-inflammatory drugs, bisphosphonates,
radiation therapy and surgery and, importantly, should direct future, more
effective ones.
