Immigrated Canadians make up approximately 20% of the total population in Canada, and 30% of the population in Ontario. Despite universal health coverage and an equal prevalence of severe arthritis in immigrants relative to non-immigrants, the former may be underrepresented amongst arthroplasty recipients secondary to challenges navigating the healthcare system. The primary aim of this study was to determine if utilization of arthroplasty differs between immigrant populations and persons born in Canada. The secondary aim was to determine differences in outcomes following total hip and knee arthroplasty (THA and TKA, respectively). This is a retrospective population-based cohort study using health administrative databases. All patients aged ≥18 in Ontario who underwent their first primary elective THA or TKA between 2002 and 2016 were identified. Immigration status for each patient was identified via linkage to the ‘Immigration, Refugee and Citizenship Canada’ database. Outcomes included all-cause and septic revision surgery within 12-months, dislocation (for THA) and total post-operative case cost and were compared between groups. Cochrane-Armitage Test for Trend was utilized to determine if the uptake of arthroplasty by immigrants changed over time. There was a total of 186,528 TKA recipients and 116,472 THA recipients identified over the study period. Of these, 10,193 (5.5%) and 3,165 (2.7%) were immigrants, respectively. The largest proportion of immigrants were from the Asia and Pacific region for those undergoing TKA (54.0%) and Europe for THA recipients (53.4%). There was no difference in the rate of all-cause revision or septic revision at 12 months between groups undergoing TKA (p=0.864, p=0.585) or THA (p=0.527, p=0.397), respectively. There was also no difference in the rate of dislocations between immigrants and people born in Canada (p=0.765, respectively). Despite having similar complication rates and costs, immigrants represent a significantly smaller proportion of joint replacement recipients than they represent in the general population in Ontario. These results suggest significant underutilization of surgical management for arthritis among Canada's immigrant populations. Initiatives to improve access to total joint arthroplasty are warranted

Introduction. The purpose of this study was to analyse the efficacy and complications associated with the use of Calcium Sulphate synthetic bone graft in a paediatric population. There are no published articles on the use in children. Materials & Methods. A retrospective review was undertaken of the notes, microbiology, and X-Rays of 17 cases (in 15 patients) of calcium sulphate use in paediatric patients. As well as patient demographic data, data collected included indication, use of additional agents (antibiotics), return to theatre, and wound complications. Major complications were also assessed for. Results. There were 17 cases, in 15 patients, in our case series where calcium sulphate synthetic bone graft was used. The average patient age was 12.0 years (range 5 years – 17 years). Indications for use included likely infection (12), possible infection (3), and 2 elective finger cases (enchondroma and osteotomy). The humerus was the most common target site (5), followed by the femur (4), tibia (3), calcaneum (2), finger (2) and metatarsal (1) also included. There were positive intra-operative microbiology samples for eleven cases (Staphylococcus aureus and Staphylococcus epidermidis). Antibiotics were used in all cases except the elective finger surgery, and choice ranged between vancomycin, gentamicin, or a combination of both. Two patients required return to theatre for management of ongoing deep infection, although one case was later deemed to be non-infective osteomyelitis. Seven patients had undergone debridements prior to the definitive one with calcium sulphate (5 without Calcium Sulphate, 2 with Calcium Sulphate). Three patients experienced wound issues in the form of discharge/leakage, all were managed with dressings and did not require return to theatre. Conclusions. Calcium Sulphate synthetic bone graft, with addition of antibiotics, is an efficacious treatment in the paediatric population and is not associated with any major complications. Wound discharge should be observed for, and patients/parents warned about this, but only as per the adult population

The opposable thumb is one of the defining characteristics of human anatomy and is involved in most activities of daily life. Lack of optimal thumb motion results in pain, weakness, and decrease in quality of life. First carpometacarpal (CMC1) osteoarthritis (OA) is one of the most common sites of OA. Current clinical diagnosis and monitoring of CMC1 OA disease are primarily aided by X-ray radiography; however, many studies have reported discrepancies between radiographic evidence of CMC1 OA and patient-related outcomes of pain and disability. Radiographs lack soft-tissue contrast and are insufficient for the detection of early characteristics of OA such as synovitis, which play a key role in CMC OA disease progression. Magnetic resonance imaging (MRI) and two-dimensional ultrasound (2D-US) are alternative options that are excellent for imaging soft tissue pathology. However, MRI has high operating costs and long wait-times, while 2D-US is highly operator dependent and provides 2D images of 3D anatomical structures. Three-dimensional ultrasound imaging may be an option to address the clinical need for a rapid and safe point of care imaging device. The purpose of this research project is to validate the use of mechanically translated 3D-US in CMC OA patients to assess the measurement capabilities of the device in a clinically diverse population in comparison to MRI. Four CMC1-OA patients were scanned using the 3D-US device, which was attached to a Canon Aplio i700 US machine with a 14L5 linear transducer with a 10MHz operating frequency and 58mm. Complimentary MR images were acquired using a 3.0 T MRI system and LT 3D coronal photon dense cube fat suppression sequence was used. The volume of the synovium was segmented from both 3D-US and MR images by two raters and the measured volumes were compared to find volume percent differences. Paired sample t-test were used to determine any statistically significant differences between the volumetric measurements observed by the raters and in the measurements found using MRI vs. 3D-US. Interclass Correlation Coefficients were used to determine inter- and intra-rater reliability. The mean volume percent difference observed between the two raters for the 3D-US and MRI acquired synovial volumes was 1.77% and 4.76%, respectively. The smallest percent difference in volume found between raters was 0.91% and was from an MR image. A paired sample t-test demonstrated that there was no significant difference between the volumetric values observed between MRI and 3D-US. ICC values of 0.99 and 0.98 for 3D-US and MRI respectively, indicate that there was excellent inter-rater reliability between the two raters. A novel application of a 3D-US acquisition device was evaluated using a CMC OA patient population to determine its clinical feasibility and measurement capabilities in comparison to MRI. As this device is compatible with any commercially available ultrasound machine, it increases its accessibility and ease of use, while proving a method for overcoming some of the limitations associated with radiography, MRI, and 2DUS. 3DUS has the potential to provide clinicians with a tool to quantitatively measure and monitor OA progression at the patient's bedside

Aboriginal and Torres Strait Islander (ATSI) people have higher elective wait times compared to non-ATSI population in Australia. The Murrumbidgee Local Health District (MLHD) in southern New South Wales services 125,242km. 2. and a population of 287,000 people, with 5.8% identifying as ATSI. The aim of this study is to investigate the arthroplasty waitlist time of ATSI, and the impact of rurality on joint replacement, within the MLHD and compared to the Australian national data. 1435 consecutive patients who underwent elective hip or knee arthroplasty from July 2018 to June 2021 were collated. Demographics, ATSI status, total wait time, readiness for care, and rurality were collected. Rurality was measured by distance from the arthroplasty hospital within MLHD. 1,151 patients were included after excluding patients with missing data or underwent emergent surgery. Within this cohort, 72 of 1,151 patients (6.2%) identified as ATSI. ATSI were younger than non-ATSI population (60.7y v 66.4y). There was no difference between Aboriginal status and ready for care wait time (368.0 v 349.9 days; p=0.116). The rurality of the groups was similar and increasing rurality did not affect total wait time (ATSI 103.1km v 98.6km; p=0.309). There was no difference in total or not-ready-for-care time between the groups (p: 0.68). Findings suggest equitable access to joint arthroplasty in the MLHD between ATSI and non-ATSI populations, which differs from the national experience. There is no significant difference between rurality and accessibility in the MLHD. This may be a result of the increased focus to ATSI and rural health within the district. A state or national study would be beneficial in identifying high performing regions and reviewing processes that enable equitable and accessible care. MLHD provides equitable access to arthroplasty surgery between ATSI and non-ATSI, as well as patients from rural areas within the LHD

Worldwide, most spine imaging is either “inappropriate” or “probably inappropriate”. The Choosing Wisely recommendation is “Do not perform imaging for lower back pain unless red flags are present.” There is currently no detailed breakdown of lower back pain diagnostic imaging performed in New Brunswick (NB) to inform future directions. A registry of spine imaging performed in NB from 2011-2019 inclusive (n=410,000) was transferred to the secure platform of the NB Institute for Data, Training and Research (NB-IRDT). The pseudonymized data included linkable institute identifiers derived from an obfuscated Medicare number, as well as information on type of imaging, location of imaging, and date of imaging. The transferred data did not include the radiology report or the test requisition. We included all lumbar, thoracic, and complete spine images. We excluded imaging related to the cervical spine, surgical or other procedures, out-of-province patients and imaging of patients under 19 years. We verified categories of X-ray, Computed Tomography (CT), and Magnetic Resonance Imaging (MRI). Red flags were identified by ICD-10 code-related criteria set out by the Canadian Institute for Health Information. We derived annual age- and sex-standardized rates of spine imaging per 100,000 population and examined regional variations in these rates in NB's two Regional Health Authorities (RHA-A and RHA-B). Age- and sex-standardized rates were derived for individuals with/without red flag conditions and by type of imaging. Healthcare utilization trends were reflected in hospital admissions and physician visits 2 years pre- and post-imaging. Rurality and socioeconomic status were derived using patients’ residences and income quintiles, respectively. Overall spine imaging rates in NB decreased between 2012 and 2019 by about 20% to 7,885 images per 100,000 people per year. This value may be higher than the Canadian average. Females had 23% higher average imaging rate than males. RHA-A had a 45% higher imaging rate than RHA-B. Imaging for red flag conditions accounted for about 20% of all imaging. X-rays imaging accounted for 67% and 75% of all imaging for RHA-A and RHA-B respectively. The proportions were 20% and 8% for CT and 13% and 17% for MRI. Two-year hospitalization rates and rates of physician visits were higher post-imaging. Females had higher age-standardized hospitalization and physician-visit rates, but the magnitude of increase was higher for males. Individuals with red flag conditions were associated with increased physician visits, regardless of the actual reason for the visit. Imaging rates were higher for rural than urban patients by about 26%. Individuals in the lowest income quintiles had higher imaging rates than those in the highest income quintiles. Physicians in RHA-A consistently ordered more images than their counterparts at RHA-B. We linked spine imaging data with population demographic data to look for variations in lumbar spine imaging patterns. In NB, as in other jurisdictions, imaging tests of the spine are occurring in large numbers. We determined that patterns of imaging far exceed the numbers expected for ‘red flag’ situations. Our findings will inform a focused approach in groups of interest. Implementing high value care recommendations pre-imaging ought to replace low-value routine imaging

Abnormal patella height has been found to be one of the main reasons for abnormal contact between patella and trochlear groove leading to patellar instability in children. Many methods have been described to diagnose patellar instability but most of them are justified only in adults. The reason being incomplete ossification in the paediatric population. These methods have been divided into direct and indirect methods. We analysed the MRI scan of knee of 57 children between 12–14 years of age with no previous diagnosis of patellar instability. Patients with a diagnosis of patellar instability, previous surgery on the knee or trauma and poor MRI scan were excluded from the study. We used Insall -Salvati Index (ISI), Caton-Deschamp Index (CDI) and Patella-Trochlear Index (PTI) and compared the results. We found that 40% of measurements by CDI and 41% by ISI showed patella alta in patients with normal patella height. 10% of patients in PTI readings had value suggestive of abnormal patella height. We concluded that PTI is a more reliable index to be used in children as it uses the length of articular surface and does not rely on bony landmarks. Studies done show PTI is a more reliable and accurate method of measuring patella height

In the vast majority of patients, the anatomical and mechanical axes of the tibia in the coronal plane are widely accepted to be equivalent. This philosophy guides the design and placement of orthopaedic implants within the tibia and in both the knee and ankle joints. However, the presence of coronal tibial bowing may result in a difference between these two axes and hence cause suboptimal placement of implanted prostheses. Although the prevalence of tibial bowing in adults has been reported in Asian populations, to date no exploration of this phenomenon in a Western population has been conducted. The aim of this study was to quantify the prevalence of coronal tibial bowing in a Western population. This was an observational retrospective cohort study using anteroposterior long leg radiographs collected prior to total knee arthroplasty in our high volume arthroplasty unit. Radiographs were reviewed using a Picture Archiving and Communication System. Using a technique previously described in the literature for assessment of tibial bowing, two lines were drawn, each one third of the length of the tibia. The first line was drawn between the tibial spines and the centre of the proximal third of the tibial medullary canal. The second was drawn from the midpoint of the talar dome to the centre of the distal third of the tibial medullary canal. The angle subtended by these two lines was used to determine the presence of bowing. Bowing was deemed significant if more than two degrees. The position of the apex of the bow determined whether it was medial or lateral. Measurements were conducted by a single observer and 10% of measurements were repeated by the same observer and also by two separate observers to allow calculation of intraclass correlation coefficients (ICCs). A total of 975 radiographs consecutively performed in the calendar years 2015–16 were reviewed, 485 of the left leg and 490 of the right. In total 399 (40.9%) tibiae were deemed to have bowing more than two degrees. 232 (23.8%) tibiae were bowed medially and 167 (17.1%) were bowed laterally. The mean bowing angle was 3.51° (s.d. 1.24°) medially and 3.52° (s.d. 1.33°) laterally. Twenty-three patients in each group (9.9% medial/13.7% lateral) were bowed more than five degrees. The distribution of bowing angles followed a normal distribution, with the maximal angle observed 10.45° medially and 9.74° laterally. An intraobserver ICC of 0.97 and a mean interobserver ICC of 0.77 were calculated, indicating excellent reliability. This is the first study reporting the prevalence of tibial bowing in a Western population. In a significant proportion of our sample, there was divergence between the anatomical and mechanical axes of the tibia. This finding has implications for both the design and implantation of orthopaedic prostheses, particularly in total knee arthroplasty. Further research is necessary to investigate whether prosthetic implantation based on the mechanical axis in bowed tibias results in suboptimal implant placement and adverse clinical outcomes

Introduction. A good anatomic fit of a Total Knee Arthroplasty is crucial to a good clinical outcome. The big variability of anatomies in the Asian and Caucasian populations makes it very challenging to define a design that optimally fits both populations. Statistical Shape Models (SSMs) are a valuable tool to represent the morphology of a population. The question is how to use this tool in practice to evaluate the morphologic fit of modern knee designs. The goal of our study was to define a set of bone geometries based on SSMs that well represent both the Caucasian and the Asian populations. Methods. A Statistical Shape Model (SSM) was built and validated for each population: the Caucasian Model is based on 120 CT scans from Russian, French, German and Australian patients. The Asian Model is based on 80 CT scans from Japanese and Chinese patients. We defined 7 Caucasian and 5 Asian bone models by using mode 1 of the SSM. We measured the antero-posterior (AP) and medio-lateral (ML) dimensions of the distal femur on all anatomies (input models and generated models) to check that those bone models well represent the studied population. In order to cover the whole population, 10 additional bone models were generated by using an optimization algorithm. First, a combined Asian-Caucasian SSM was generated of 92 patients, equally balanced between male and female, Caucasian and Asian. 10 AP/ML dimensions were defined to obtain a good coverage of the population. For a given AP/ML dimension, Markov chain Monte Carlo sampler was used to find the most average shape with AP/ML dimensions as close as possible to the target dimensions. The difference of the AP/ML dimensions of the generated models to the target dimensions was computed. A chi-squared distribution was used to assess how average the resulting shapes were compared to typical patient shapes. Results. The AP-ML dimensions of the 7 Caucasian bones and the 5 Asian bones well cover the range of the respective populations. For the Caucasian Femur, the AP/ML dimensions range from (53,6/64,9mm) for size 1 to (67,7/80,7mm) for size 7. For the Asian Femur, the AP/ML dimension range from (53,0/62,4mm) for size 1 to (60,5/72,4mm) for size 5. The dimensions of the 10 additionally generated bones differed in average (± 1 standard deviation) by 0,2±0,4mm in AP and 0,5±0,5mm in ML to the target dimensions. The maximal deviation was 0,9mm in AP and 1,0mm in ML. All 10 bones had a P-value of P < 10. -27. according to the chi-squared distribution. Conclusion. The proposed models of 7 Caucasian and 5 Asian bones well represent both populations. The 10 additional geometries enable to get a complete coverage of the population. Since they are very close to average, all these bone models provide more generalized reference shapes compared to individual patients. By performing a virtual implantation on those anatomies, the anatomical fit of implants to these populations can be evaluated. For any figures or tables, please contact authors directly

Aims. This study aims to define the epidemiology of trauma presenting to a single centre providing all orthopaedic trauma care for a population of ∼ 900,000 over the first 40 days of the COVID-19 pandemic compared to that presenting over the same period one year earlier. The secondary aim was to compare this with population mobility data obtained from Google. Methods. A cross-sectional study of consecutive adult (> 13 years) patients with musculoskeletal trauma referred as either in-patients or out-patients over a 40-day period beginning on 5 March 2020, the date of the first reported UK COVID-19 death, was performed. This time period encompassed social distancing measures. This group was compared to a group of patients referred over the same calendar period in 2019 and to publicly available mobility data from Google. Results. Orthopaedic trauma referrals reduced by 42% (1,056 compared to 1,820) during the study period, and by 58% (405 compared to 967) following national lockdown. Outpatient referrals reduced by 44%, and inpatient referrals by 36%, and the number of surgeries performed by 36%. The regional incidence of traumatic injury fell from 5.07 (95% confidence interval (CI) 4.79 to 5.35) to 2.94 (95% CI 2.52 to 3.32) per 100,000 population per day. Significant reductions were seen in injuries related to sports and alcohol consumption. No admissions occurred relating to major trauma (Injury Severity Score > 16) or violence against the person. Changes in population mobility and trauma volume from baseline correlated significantly (Pearson’s correlation 0.749, 95% CI 0.58 to 0.85, p < 0.001). However, admissions related to fragility fractures remained unchanged compared to the 2019 baseline. Conclusion. The profound changes in social behaviour and mobility during the early stages of the COVID-19 pandemic have directly correlated with a significant decrease in orthopaedic trauma referrals, but fragility fractures remained unaffected and provision for these patients should be maintained. This study aimed to evaluate the clinical outcomes of paediatric patients who underwent a retrograde drilling treatment for their osteochondritis dissecans (OCD) of the talus. The secondary purpose was to identify factors that are predictive of a failure of the treatment. A retrospective study was done. All patients treated for talar OCD between 2014 and 2017 were reviewed to extract clinical and demographic information (age, sex, BMI, OCD size and stability, number of drilling, etc). Inclusion criteria were: (1) talar OCD treated with retrograde drilling, (2) less than 18 years, (3) at least one available follow up (4) stable lesion. Exclusion criteria was another type of treatment for a the talar OCD. Additionally, all pre-operative and post-operative medical imaging was reviewed. Outcome was classified based on the last follow-up appointment in two ways, first a score was attributed following the Berndt and Harty treatment outcome grading and second according to the necessity of a second surgery which was the failure group. Chi-square and Mann-Whitney tests were used to compared the success and failure group. Seventeen patients (16 girls and 1 boy, average age: 14.8±2.1 years) were included in our study group. The mean follow up duration was 11.5 (±12) months. Among this population, 4/17 (24%) had a failure of the treatment because they required a second surgery. The treatment result grading according to Berndt and Harty outcome scale identified good results in 8/17 (47%) patients, fair results in 4/17(24%) patients and poor results in 5/17 (29%) patients. The comparisons for various patient variables taken from the medical charts between patients who had a success of the treatment and those who failed did not find any significant differences. At a mean follow-up duration of 11.5 months, 76% of patients in this study had a successful outcome after talar OCD retrograde drilling. No statistically significant difference was identified between the success and failure group. Talar OCD in a paediatric population is uncommon, and this study reviewed the outcome of retrograde drilling with the largest sample size of the literature. Retrograde drilling achieved a successful outcome in 76% of the cases and represents a good option for the treatment of stable talar OCD

The meniscus is at the cornerstone of knee joint function, imparting stability and ensuring shock absorption, load transmission, and stress distribution within the knee joint. However, it is very vulnerable to injury and age-related degeneration. Meniscal tears are reported as the most common pathology of the knee with a mean annual incidence of 66 per 100,000. Knee osteoarthritis progresses more rapidly in the absence of a functional meniscus. Historically, tears extending to the avascular inner portion of the meniscus (white-white zone, “WW”), such as radial tears were considered as untreatable and were often resected, due to the lack of vascularity in the WW zone. Perfusion-based anatomical studies performed on cadaveric menisci in the 1980s shaped the current dogma that human meniscus has poor regenerative capacity, partly due to limited blood supply that only reaches 10 to 25% of the meniscus, commonly referred to as red-red zone (“RR”). Previous studies, including those utilizing animal models have shown mobilization of Mesenchymal Stem Cells (MSCs) upon injury into the WW zone, and successful MSC recruitment when administered externally to the injury site. We and others have recently reported positive outcomes of repaired tears in the inner zone of patients. We hypothesized that the “avascular” white-white zone of the meniscus possesses regenerative capacity due to a resident stem/progenitor cell population. Further, we sought to redefine the presence of microvessels in all meniscal zones using advanced stereology and imaging modalities. Fifteen menisci from fresh human cadaveric knees (mean age: 21.53±6.53 years) without evidence of previous injury were obtained from two tissue banks (JRF, Centennial, CO) and Biosource Medical (Lakeland, FL) and utilized for this study. The use of cadaveric specimens for research purposes was approved by the institutional review board. Tibial plateaus were dissected to harvest medial and lateral menisci along their entire length. The RR, red-white (RW) and WW zones were dissected and separated into three thirds from the inner aspect to the marginal border of the meniscus and their wet weights recorded (Fig.1A). Meniscus tissue cellular content in each zone was obtained from dissociation of meniscus tissue using 0.02% w/v pronase (Millipore) for 1h at 37oC, followed by 18h 0.02% w/v collagenase II (Worthington) at 37oC with shaking. Isolated cells were characterized immediately after harvest using flow cytometry with antibodies against MSCs surface markers (CD105, CD90, CD44 and CD29) as well as respective isotype controls. Further, meniscal cells were cultured and split twice when confluence was reached, characterized at P2 and compared to bone marrow-derived MSCs (BM-MSCs) using the same markers. Self-renewal of cells was assessed using colony forming unit (CFU) assay. Differentiation assays were performed to assess whether colony-forming cells retained multilineage potential. For morphological examination of bigger vessels, samples were fixed in 10% formalin for 1 week, paraffin embedded, sectioned (4 μm thick) and stained with H&E and Masson's trichrome. Presence of microvessels was assessed by CD31 immunofluorescence staining. Further, menisci were cleared using the uDisco protocol labeled with the TO-PRO®-3 stain, a fluorescent dye that stains cell nuclei and imaged using light-sheet microscopy. All continuous data are presented as mean ±standard deviation. Non-repeated measures analysis of variance (ANOVA) and Tukey-Kramer HSD post hoc analysis were performed on sample means for continuous variables. Statistical significance was set at p < 0 .05. Menisci were successfully cleared using a modified uDISCO procedure, imaged and analyzed for total cell density. As expected, bigger vessels were observed in RR but not in WW. However, immunofluorescent staining for CD31 showed a subset of CD31+endothelial cells present in the WW zone, indicating the presence of small vessels, most likely capillaries. In order to assess whether enzymatic digestion had a differential result depending on meniscus zone due to cellular content, we analyzed yields per meniscus per zone. The wet weight of different zones (WW:RW:RR) was at a ratio of ∼1:3:5 respectively, however, the ratio of cells isolated from each zone was at ∼1:4:20, indicating that RR has a denser population of mononuclear cells. However, the difference between all zones in cell yields was not significant. The clonogenic potential of isolated cells was shown to be non-significantly different between the three zones. Differentiation of isolated cells to osteogenic lineage using osteogenic media in vitroshowed no difference between the three zones. Flow cytometry analysis of cells from the three meniscal zones displayed presence of two distinct subpopulations of cells immediately after isolation. One subpopulation was positive to MSC surface markers and the other negative. Additionally, flow cytometry of cultured meniscal cells at P2 displayed that the entire cell population was CD44+CD105+CD29+CD90+, suggesting that culturing meniscal cells results in selection of stem/progenitor cells (plastic adherence). Surface marker expression analysis showed differential expression patterns between markers depending on zone. Similar fraction of cells was detected to express both MSC markers CD90 and CD105 (7–10%) and similar fraction of cells expressed both MSC markers CD29 and CD44 (1–2%) in all three zones, indicating similar density of resident stem/progenitor cells in each zone. Importantly, WW showed significantly higher expression for all four MSC markers compared to the RR zone, indicating higher relative density of stem/progenitor resident cells in the WW zone. Our results determine that CD31-expressing microvessels were present in all zones, including the WW zone, which was previously considered completely avascular. Additionally, stem/progenitor cells were shown to be present in all three zones of the menisci, including the WW zone, showcasing its regenerative potential. Introduction. One of the objectives of total hip arthroplasty is to restore femoral and acetabular combined anteversion. It is desirable to reproduce both femoral and acetabular antevesions to maximize the acetabular cup fixation coverage and hip joint stability. Studies investigated the resultant of implanted femoral stem anteversion in western populations showed that the implanted femoral stems had only a small portion can meet the desirable femoral anteversion angle. 1. , and anteversion angle increases after the implantation of an anatomical femoral stem with anteverted stem neck comparing to anatomical femoral neck. 2. The purpose of this study was to anatomically measure the anteversion angular difference between metaphyseal long axis and femoral neck in normal Chinese population. The metaphyseal long axis represents the coronal fixation plane of modern cementless medial-lateral cortical fitting taper stem. This angular difference or torsion Δ angle provides the estimation of how much the neck antevertion angle of femoral stem would be needed to match for desirable anatomical femoral neck version. Methods. 140 (77 male and 63 female) anonymous normal adult Chinese CT data with average age of 54.6 (male 54.6, female 54.5, P=0.95) were segmented and reconstructed to 3D models in Trauson Orthopeadic Modeling and Analytics (TOMA) program. Femoral head center, femoral neck axis and center point of diaphyseal canal 100mm bellow calcar formed the femoral neck plane. The metaphyseal stem implantation plane was determined by the center point of medial calcar, proximal canal central axis formed by femoral neck plane and the center point of diaphyseal canal 100mm bellow calcar. [Fig. 1] The angle between two planes was the torsion Δ angle between femoral placement plane and anatomical femoral neck. [Fig. 2] The torsion Δ angles were measured for all 140 cases. The traditional anteversion angle for anatomical femoral neck was also measured by Murphy's method. Student T test was perform to compare the angles for male and female. The 98% confidence level was assumed. Results. The average torsion Δ angle for whole population was 4.9°(0.04°-15.6°), SD=3.52°, male: 4.6° (0.42°-13.9°), SD=3.09°; female: 5.3° (0.04°-15.6°), SD=3.98°. There was no statistical significant difference between genders. P=0.28. All metaphyseal stem placement planes were less anteverted than anatomical femoral neck plane. [Fig. 3] The average anatomical femoral neck anteversion angle for total population was 18.6° (0.27°-42.6°), SD=7.54°; male: 18.6° (0.27°-32.9°), SD=7.37°; female: 18.7° (1.74°-42.6°), SD=7.81°. There was no statistical significance between male and female P=0.92. Only 26% of study population or 37 cases with unadjusted implant neck version had normal anteversion angle of 10°-15° (Tönnis). Discussion. The study suggested femoral stem neck anteversion angle adjustments up to 11° was necessary to match anatomical femoral neck for 94% of cases in Chinese population. And the adjustments of 0°-7° represented the 76% majority of population. This finding was in agreement with the published data in western population. 2. . Significance. Variable femoral stem neck anteversion angles up to 11° are necessary to reproduce the anatomical anteversions for 94% of normal Chinese population. Malnutrition is often associated with the advanced age and can be influenced by physical, mental, social and environmental changes. Hip fracture is a major issue and a prior poor nutritional status is associated with higher rates of perioperative complications and prolonged hospital length of stay. Prospective observational cohort study performed in a Level one trauma center including 189 consecutive patients admitted for hip fracture. The main outcome measure was the Mini Nutritional Assessment (MNA), a specific tool validated for geriatric population. This questionnaire was performed at admission by an independent assessor, at the same time as a large set of demographic and functional data. Blood samples were tested for blood count and albuminemia. Two groups were constituted and analysed according to a MNA score ≥ 24 (lower limit for normal nutritional status). Factors explored included physical and mental items. Impact of malnutrition was determined on hospital length of stay (HLS), discharge in an adverse location than prior to admission (DAL), complications and mortality rate. The rate of patients with malnutrition (or at risk) in this study is 47% (88 patients). Patients with a MNA < 24 are older (84.81 yrs ± 7.75 vs 80.41 ± 8.11, p<0,01), have more comorbidities (Charlson 2.8 ± 2.21 vs 1.67 ± 3.10, p<0,01), a more impaired mental (MMSE 19.39±8.55 vs 25.6±3.6, p<0,01) or physical status (MIF 105.3 ± 26.6 vs 121.8 ± 6.4, p< 0,01). Blood samples are not selective to detect malnutrition (p=0,64). Malnutrition is associated with a longer HLS (26.04±23.39 days vs 13.95±11.34 days, p<0,01), a greater DAL (58.9% vs 38.2%, p=0,02) and a higher one year mortality rate (23.9% vs 8.9 %, p<0,01). The prevalence of malnutrition in a geriatric population admitted for hip fracture is high. Blood samples at admission have clearly a poor value and a systematic screening with the MNA is mandatory. An early diagnosis will target specific interventions to reduce the physical and socio-economic impact of the malnutrition. Future studies should focus on actions in the perioperative stage (fast-track surgery, nutritional protocols, analgesia) and their impact on the socio-economic burden

Short-stem total hip arthroplasty (THA) has primarily been recommended for young and active patients, mainly due to its bone preserving philosophy. Elderly patients, however, may also benefit of a minimally invasive technique due to the short and curved implant design. The purpose of this study was to compare the clinical and radiological outcomes as well as perioperative complications of a calcar-guided short stem between a young (75 years) population. Data were collected in a total of 5 centers, and 400 short stems were included as part of a prospective multicentre observational study between 2010 and 2014 with a mean follow-up of 49.2 months. Clinical and radiological outcomes were assessed in both groups. Secondary outcomes such as perioperative complications, rates and reasons for stem revision were also investigated. No differences were found for the mean visual analogue scale (VAS) values of rest pain, load pain, and satisfaction. Harris Hip Score (HHS) was found to be slightly better in the young group. Comparing both groups, no statistically significant differences ere found in the radiological parameters that were assessed (stress-shielding, cortical hypertrophy, radiolucency, osteolysis). Aseptic loosening was the main cause of implant failure in younger patients whereas in elderly patients, postoperative periprosthetic fractures due to accidental fall was found to be the main cause for stem revision. These short-term results are encouraging towards the use of a cementless short stem in the geriatric population. According to our findings, advanced age and potentially reduced bone quality should not necessarily be considered as contra-indications for calcar-guided short-stem THA but careful and reasonable selection of the patients is mandatory. Longer follow up is necessary in order to draw safer conclusions

‘Getting It Right First Time’ guidance recommends the universal use of cemented prostheses in patients aged over 65 within the UK. This cut off has since been raised to 70. The report claims that the increased cost of uncemented technology is not justifiable in terms of patient outcomes. Our aim was to evaluate any disparity between patients across these age thresholds, in terms of functional outcomes, complication and costs following elective uncemented THR.

We utilised a single surgeon prospectively updated database, to compare functional outcome of patients aged over and under 65 and again at 70 following elective uncemented THR. We measured functional outcome using Oxford Hip Score (OHS). Patients were followed routinely for up to 2 years and subsequently up to 6 years.

Patient ages ranged from 23–89. Over 97% of patients reported an improvement in hip function, with an average increase of 24.1 in OHS. There was no statistical significance between patient age and functional hip outcome, p=0.108 with a cut off at 65 and p=0229 at 70. 1.1% of under 65s required revision surgery, compared to 2.3% of over 65s. 1.4% of patients under 70 required revision surgery, compared to 2.5% in those above. The most common reason for revision surgery was debridement, antibiotics and implant retention. Patients under 65 had a 1.4% chance of experiencing a fracture or dislocation, compared to 2.5% of patients over 65. 2.0% of patients under 70 experienced a fracture or dislocation, in comparison to 2.2% in those over 70. There was 1 recorded mortality, a patient aged over 70. Cost analysis is challenging accounting for all variables between techniques. Cementing invariably requires additional operating time and diminished theatre efficiency, with direct cost estimated at £364 in theatre running fees per case.

Within the series, uncemented THRs was found to be a safe and efficacious procedure irrespective of age. Functional hip outcome was not correlated to patient age, conflicting with GIRFT recommendations. The extra duration of cemented surgery may equilibrate the financial disparity of uncemented practice.

The purpose of this study was to define the risk and incidence of eventual ankle arthroplasty or fusion after documented ankle fracture in a large cohort, and compare that rate to matched healthy patients from the general population. The Ontario health insurance plan (OHIP) physician billing database, Institute for Clinical Evaluative Sciences (ICES) Physician Database, the Canadian Institute for Health Information (CIHI) databases, Discharge Abstract Database (DAD) and Same Day Surgery (SDS) were used to identify patients treated surgically and non-surgically for ankle fractures. Each patient was matched to four individuals from the general population (13.5 million) with no documented prior treatment for ankle fracture, according to age, sex, income, and urban/rural residence. Fusion and replacement incidence was compared using time-to-event analysis (Kaplan-Meier). A Cox Proportional Hazards model was used to explore the influence of patient, provider and surgical factors on time to surgery. We identified 45,444 (58.8% female, mean age 48.7 years) and 140, 629 (53.9% female, mean age 47.1 years) patients who had undergone open reduction internal fixation (ORIF) or non-operative management of an ankle fracture (NOA), respectively. Among ORIF patients, n=237 (0.5%) and n=69 (0.15%) patients underwent fusion or arthroplasty after a median 2.8 and 6.9 years, respectively. Among non-operatively treated ankle fractures, n=198 (0.14%) and n=36 (0.03%) patients underwent fusion or arthroplasty after a median of 3.2 and 5.6 years, respectively. Surgical treatment (vs. non-operatively treated fracture), older age, greater co-morbidity and a history of infection post fracture significantly increased the risk of eventual fusion or arthroplasty (HR 3.6 (3.1–4.3), p < 0 .001, HR 1.01 (1.01–1.02), p=0.009, HR 1.2 (1.1–1.3), p < 0 .001, HR 11.3 (6.8–18.7), p < 0 .001, respectively). Compared to matched controls, the risk of fusion/arthroplasty was not independent of time, following an exponential decay pattern. ORIF patient risk was 20 times greater than the general population in the first three years post-ORIF, and approached the risk of non-operatively treated patients (HR 4.5 (95CI: 3.5–5.8), p < 0 .0001) by approximately 14 years out from injury on time and comorbidity adjusted KM curves. Rates of fusion/arthroplasty are very low after ORIF and non-operative treatment of an ankle fracture in the general population of a public healthcare system. Utilization patterns suggest fusion is more common earlier, and arthroplasty remote, which may be a factor of patient age, injury severity, and complications from initial injury/surgery. Patients who underwent ORIF have >20 times the risk of fusion/arthroplasty in the short-term, however, the risk decreases over time eventually approaching that of non-operatively treated patients (∼4.5x the general population) when compared to non-fractured controls

The inmate population is a unique cohort with several healthcare-related challenges. International studies have demonstrated higher rates of infectious diseases, chronic diseases and psychiatric disorders in inmates when compared to general population. However, little is known about the outcomes following total joint arthroplasty in this population. This retrospective chart review aims to outline the differences in clinical outcomes after hip and knee total joint arthroplasty in the Kingston inmate population compared to the national population standard. A list of all inmate inpatient hospital visits with diagnostic/procedure codes pertaining to total joint arthroplasty within the last ten years was obtained through a computer-based search of the Kingston General Hospital Discharge Abstract Database(DAD). The patient charts were reviewed and demographic and outcome data pertinent to our study was collected. Data was compiled using Excel and imported into IBM SPSS for descriptive analysis. Twenty male inmate patients underwent 24 primary Total Hip Arthroplasties(THA) or Total Knee Arthroplasties(TKA) and one medial unicompartmental knee arthroplasty from May 2003 to January 2013. The average age was 58 with mean Body Mass Index(BMI) of 34. Median American Society of Anesthesiologist(ASA) score was 3 and mean Charlston Comorbidity Index was 3.92. The rates of HCV and HIV were 35%(n=5) and 0%, respectively. Average length of stay from time of initial procedure was 4.2 days. The overall revision rate was 24% (n=6). Reasons for revision included deep prosthetic infection (50%, n=3), aseptic loosening (17%, n=1), arthrofibrosis (17%, n=1) and late periprosthetic fracture (17%, n=1). Infection rates were reported at 16% (n=4); 75% of which were deep prosthetic infections requiring revision surgery. Other complications included ST-elevation myocardial infarction(STEMI) (n=1), and postoperative knee stiffness requiring manipulation under anesthesia(MUA) (n=1). Compared to Correctional Services Canada(CSC) data on male inmate health in 2012, our study population demonstrated a higher rate of HCV (35% vs. 9.4%), diabetes (30% vs. 4.2%) and overall cardiovascular and respiratory comorbidities. This may reflect the higher rate of comorbidities associated with osteoarthritis, such as BMI>25, which was evident in 95% of our study population vs. 64.5% in Canadian inmates. Total joint revision rates in our inmate study population was 24%, which is higher than the 2014 Canadian Joint Replacement Registry's yearly revision rate of 8.7% in THA and 5.2% in TKA. Our study population also demonstrated infection as the leading cause for revision at 50%, compared to 14.5% for THA and 19.6% for TKA in the general Canadian population. Further study of the complex biopsychosocial risk factors in the inmate population is warranted to better define pre-surgical risk assessment criteria

Introduction. Appropriate femoral stem anteversion is an important factor in maintaining stability and maximizing the performance of the bearing after total hip replacement (THR). The anteversion of the native femoral neck has been shown to have a significant effect on the final anteversion of the stem, particularly with a uncemented femoral component. The aim of this study was to quantify the variation in native femoral neck anteversion in a population of patients requiring total hip replacement. Methods. Pre-operatively, 1215 patients received CT scans as part of their routine planning for THR. Within the 3D planning, each patient's native femoral neck anteversion, measured in relation to the posterior condyles of the knee, was determined. Patients were separated into eight groups based upon gender and age. Males and females were divided by those under 55 years of age, those aged 55 to 64, 65 to 74 and those 75 or older. Results. The median anteversion in males was 12.7° (−27.1°–45.5°, IQR 6.0°–19.1°), compared to female anteversion of 16.0° (− 14.0°–54.5°, IQR 9.7°– 22.4°). These gender differences were statistically significant, p < 0.0001. Femoral anteversion in young males (<55) was significantly higher than in older males (>75), p=0.002. This age-related difference approached significance in females, p = 0.06. 14% of patients had extreme anteversion (<0° or >30°). Conclusions. The Native femoral neck anteversion in patients requiring THR is widely variable, with a range of over 80°. Females have more anteverted femurs than males. Femoral anteversion in young males was significantly higher than in older males. This age-related difference approached significance in females. Having an understanding of 3D patient morphology can greatly assist in pre-operative planning of THR, as post-op stem anteversion is likely influenced by the anteversion of the native femoral neck

Total knee arthroplasty is a successful procedure that reduces knee pain and improves function in most patients with knee osteoarthritis. Patient dissatisfaction however remains high, and along with implant longevity, may be affected by component positioning. Surgery in obese patients is more technically challenging with difficulty identifying appropriate landmarks for alignment and more difficult exposure of the joint. Patient specific instrumentation (PSI) has been introduced with the goal to increase accuracy of component positioning by custom fitting cutting guides to the patient using advanced imaging. A strong criticism of this new technology however, is the cost associated. The purpose of this study was to determine, using a prospective, randomized-controlled trial, the cost-effectiveness of PSI compared to standard instrumentation for total knee arthroplasty in an obese patient population. Patients with a body mass index greater than 30 with osteoarthritis and undergoing a primary total knee arthroplasty were included in this study. We randomized patients to have their procedure with either standard instrumentation (SOC) or PSI. At 12-weeks post-surgery patients completed a self-reported cost questionnaire and the Western Ontario and McMaster Osteoarthritis Index (WOMAC). We performed a cost-effectiveness analyses from a public health payer and societal perspective. As we do not know the true cost of the PSI instrumentation, we estimated a value of $100 for our base case analysis and used one-way sensitivity analyses to determine the effect of different values (ranging from $0 to $500) would have on our conclusions. A total of 173 patients were enrolled in the study with 86 patients randomized to the PSI group and 87 to the SOC group. We found the PSI group to be both less effective and more costly than SOC when using a public payer perspective, regardless of the cost of the PSI. From a societal perspective, PSI was both less costly, but also less effective, regardless of the cost of the PSI. The mean difference in effect between the two groups was −1.61 (95% CI −3.48, 026, p=0.091). The incremental cost-effectiveness ratio was $485.71 per point increase in the WOMAC, or $7285.58 per clinically meaningful difference (15 points) in the WOMAC. Overall, our results suggest that PSI is not cost-effective compared to standard of care from a public payer perspective. From a societal perspective, there is some question as to whether the decreased effect found with the PSI group is worth the reduced cost. The main driver of the cost difference appears to be time off of volunteer work, which will need to be investigated further. In future, we will continue to follow these patients out to one year to collect cost and effectiveness data to investigate whether these results remain past 12 weeks post-surgery

Although total hip arthroplasty (THA) has been shown to be a cost-effective means of treating hip arthritis, there is some ambiguity within the literature as to its success in those over 80 years of age. With the rapid expansion of this population group and an estimated 40% rise in THA figures expected by 2026, this study aims to review the results of primary THA in the octogenarian population. A series of 510 consecutive cases was obtained from the local arthroplasty database, consisting of all patients aged 80 years and over who underwent primary THA between 1994 and 2004. A control group of 3404 individuals under 80 years was also established using the same database and inclusion criteria. Mean follow-up for the octogenarian group was 5.9 years. Pain scores were comparable five years post-operatively in both groups (P=0.479); in particular 81.5% of octogenarians and 80.2% of the control noted no pain. Pre-operatively, the mean Harris Hip Function and Harris Hip Score were significantly lower in the octogenarian group by 4.3 and 4.2 points (P< 0.001), respectively, and at five years follow-up were also lower by 8.4 and 8.0 points, respectively (P< 0.001). Median hospital stay was three days longer in the elderly population (12 cf 9, P< 0.001). More complications occurred in the octogenarian group (38.1% cf 28.7% of controls, P< 0.001) however fewer cases of revision were noted (1.4% cf 3.8%, P=0.005). Kaplan-Meier analysis found implant survival time to revision to be comparable in both groups (mean 16.4 years in control cf 14.3 years in octogenarian, P=0.17). Patient satisfaction was also similar (97.8% in octogenarians and 98.1% in controls, P=0.741). This study suggests that individuals over 80 years of age have comparable pain improvement and overall satisfaction, low revision rates, reduced functional improvement and are more prone to complications compared to younger patients
