Introduction. Distal femur fractures have traditionally been stabilized with either lateral locking plate or retrograde intramedullary nail. Dual-plates and nail-plate combination fixation have the theoretical biomechanical advantage, faster union and allows patients to weight bear immediately. The aim of this study is to compare single vs combination fixation, and evaluate outcomes and complications. Method. We retrospectively reviewed all patients over 60, admitted to Christchurch Hospital, between 1st Jan 2016 and 31st Dec 2022, with an AO 33A/33B/33C distal femur fracture. Patient demographics, fracture characteristics, operation details, and follow up data were recorded. Primary outcomes are union rate, ambulatory status at discharge, and surgical complications. Secondary outcomes include quality of reduction, operation time and rate of blood transfusions. Results. 114 patients were included. (92 single fixation, 22 combination fixation). Baseline demographic data and fracture characteristics did not differ between the cohorts. There was no difference in the rate of union or time to union between the two cohorts. Combination fixation patients were allowed to weight-bear as tolerated significantly more than single fixation patients (50% vs 18.9%, p=0.003). There was no difference in length of hospital stay, transfusion, complication and mortality rates. Medial translation of the distal articular block was significantly lower in the combination fixation cohort (1.2% vs 3.4%, p=0.021). Operation time was significantly longer in the combination fixation cohort (183mins vs 134mins, p<0.001). Discussion. The results show no difference in achieving union or time to union, despite better quality of fracture reduction with dual fixation. This differs to previously published literature. The clear benefit of combination fixation is immediate weight-bearing. As expected, operation times were longer with combination fixation, however this did not translate to more complications. Conclusion. Combination fixation allows earlier weight bearing, at the cost of longer operation times

Introduction. This study aimed to assess the relationship between preparation times and operative procedures for elective orthopaedic surgery. A clearer understanding of these relationships may facilitate list organisation and thereby contribute to improved operating theatre efficiency. Methods. Two years of elective orthopaedic theatre data was retrospectively analysed. The hospital medical information unit provided de- identified data for 2015 and 2016 elective orthopaedic cases, from which were selected seven categories of procedures with sufficient numbers to allow further analysis - primary hip and knee replacement, spinal surgery, shoulder surgery (excluding shoulder replacement), knee surgery, foot and ankle surgery (excluding ankle replacement), Dupuytrens surgery and general orthopaedic surgery. The data analysed included patient age, ASA grade, operation, operation time, and preparation time (calculated as the time from the start of the anaesthetic proceedings to the patient's admission to Recovery, with the operating time [skin incision to skin closure] subtracted). Statistical analysis of the data was undertaken. Results. A total of 1596 procedures performed over the two year period were analysed. Preparation times for the different procedures were assessed, along with the relationship to the procedure complexity. Neither age nor ASA correlated strongly with preparation times. Spine procedures had greater preparation times than hip and knee arthroplasty. Greater uniformity in preparation times for hip and knee arthroplasty was seen across the anaesthetic group than operative times across the surgeon group. Discussion. Preparation times are just one aspect that may be evaluated with regard to theatre utilisation. This study did not address the theatre turn-over time between cases, which includes transfer of the patient from the admitting/pre-operative area into the theatre. Conclusion. Preparation times for elective procedures follow a pattern which may be used to inform list planning, with the potential for greater theatre efficiencies with regard to list utilisation and staff allocation

Anterior cervical discectomy and fusion (ACDF) is a well-established spinal operation for cervical disc degeneration disease with neurological compromise. The procedure involves an anterior approach to the cervical spine with discectomy to relieve the pressure on the impinged spinal cord to slow disease progression. The prosthetic cage replaces the disc and can be inserted stand-alone or with an anterior plate that provides additional stability. The literature demonstrates that the cage-alone (CA) is given preference over the cage-plate (CP) technique due to better clinical outcomes, reduced operation time and resultant morbidity. This retrospective case-controlled study compared CA versus CP fixation used in single and multilevel anterior cervical discectomy and fusion for myelopathy in a tertiary centre in Wales. A retrospective clinico-radiological analysis was undertaken, following ACDF procedures over seven years in a single tertiary centre. Inclusion criteria were patients over 18 years of age with cervical myelopathy who had at least six-month follow-up data. SPSS was used to identify any statistically significant difference between both groups. The data were analysed to evaluate the consistency of our findings in comparison to published literature. Eighty-six patients formed the study cohort; 28 [33%] underwent ACDF with CA and 58 [67%] with CP. The patient demographics were similar in both groups, and fusion was observed in all individuals. There was no statistical difference between the two constructs when assessing subsidence, clinical complication (dysphagia, dysphonia, infection), radiological parameters and reoperations. However, a more significant percentage [43% v 61%] of patients improved their cervical lordosis angle with CP treatment. Furthermore, the study yielded that surgery to upper cervical levels results in a higher incidence of dysphagia [65% v 35%]. Finally, bony growth across the cage was observed on X-ray in 12[43%] patients, a unique finding not mentioned in the literature previously. Our study demonstrates no overall difference between the two groups, and we recommend careful consideration of individual patient factors when deciding what construct to choose

Purpose. The aim of this study was to compare the clinical outcomes of the revision TKA in which trabecular metal cones and femoral head allografts were used for large bone defect. Method. Total 53 patients who have undergone revision TKA from July 2013 to March 2017 were enrolled in this study. Among them, 24 patients used trabecular metal cones, and 29 patients used femoral head allografts for large bone defect. There were 3 males and 21 females in the metal cone group, while there were 4 males and 25 females in the allograft group. The mean age was 70.2 years (range, 51–80) in the femoral head allograft group, while it was 79.1 years (range, 73–85) in the metal cone group. Bone defect is classified according to the AORI classification and clinical outcomes were evaluated with Visual Analogue Scale (VAS), Hospital Special Surgery-score (HSS), Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC), Knee Injury and Osteoarthritis Outcome Score (KOOS), and ROM. Operation time was also evaluated. We used radiographs to check complications such as migration or loosening. We took follow-up x-rays and 3D CT of the patients, to assess the mean bone union period. Shapiro-Wilk test was done to check normality and Student T-test and Mann Whitney U-test were done for comparison between two groups. Result. The mean follow-up period was 3 .75 years (Range; 2.1 ∼ 5.75). The pre-op scores did not show significant difference. The mean VAS in the allograft and trabecular metal cone groups was 2.1 ± 0.87 and 1.8 ± 0.53, respectively (p = 0.16). The mean HSS score were 76.3 ± 5.51 and 79.2 ± 4.12 respectively (p = 0.13) and the mean WOMAC scores were 15.1 ± 3.25 and 14.8 ± 3.31 respectively (p = 0.06), and the mean KOOS scores were 27.8 ± 4.77 and 25.5 ± 4.84, respectively (p = 0.07). The mean ROM ranges were 100.6 ± 17.54 and 101.3 ± 19.22, respectively (p = 0.09). But the mean operation time of the allograft and trabecular metal cone groups was 137 minutes (Range; 111–198) and 102minutes (Range; 93 −133) (p=0.02) respectively, which showed statistical significance. In follow-up x-rays, no migration or loosening of the implants, osteolysis and other complications were found in both groups. In follow-up 3D CT, osteointegration was seen at the trabecular metal cone site, host bone being interpreted to the host bone. The allograft group showed fibrous and stable union in follow-up 3D CT. Conclusion. According to this study, in case of revision TKA with large bone defect, using whether allograft or trabecular metal cones did not affect the clinical outcomes. However, operation time was significantly shorter in trabecular metal cone group, therefore, in patients with poor general condition along with severe underlying diseases, usage of trabecular metal cone would be a better choice to shorten operation time and ease postoperative care. Background. Well-fixed cementless stems are sometimes needed to be extracted in patients with complications including periprosthetic infection, stem-neck breakage or trunnionosis. The purpose of this study was to report the clinical outcome in patients undergoing re-implantation surgery following removal of a well-fixed porous-coated cementless stem by the femoral longitudinal split (FLS) procedure(Fig.1, Fig.2). Methods. We conducted a retrospective study and radiographic review of 16 patients who had undergone re-implantation following the FLS procedure to remove a well-fixed stem due to periprosthetic infection, stem-neck breakage or trunnionosis. The study group consisted of 2 men and 14women with an average age of 68.4 years. Mean follow-up was 33.1± 25.0 months. Operation time, intraoperative bleeding, complications, causes of re-operation and clinical score were evaluated and the Kaplan-Meier method was used to evaluate the longevity of the stem. Results. The average operation time was 272±63 minutes and intraoperative bleeding was 420±170 ml. Although postoperative dislocation occurred in 5 hips and sinking of the stem was found in 3 hips after surgery, no progression of the stem sinking was observed and the clinical JOA and JHEQ scores were both improved after re-implantation surgery. Re-implantation surgery with Zweymüller-type stems, which are shorter than those removed, revealed evidence of osseointegration of the stem without femoral fracture. Kaplan-Meier survival analysis of stem revision for any reason as the end point revealed 70.3% survival at 9 years (Fig.3). Conclusion. The FLS procedure is expected to confer successful clinical results without loosening of the stem, following safe extraction of well-fixed porous-coated cementless stems without fracture and will allow re-implantation with shorter cementless stems than those removed. Purpose. Various approaches have been reported for the total hip replacement (THR). In recent years, a muscle sparing approach with low postoperative muscle weakness and low dislocation risk has been frequently selected. However, such surgery has a learning curve. Thus, at the time of switching from the conventional approach to such approaches, invasion or infection risk may increase with the operation time extension. The purpose of this study is to clarify the change of invasiveness or latent infection rate with the change in approach in order to select the cases safely at the beginning of introducing a new approach in THR. Methods. In facility A, THR was performed with Dall's approach (Dall), but 1 surgeon changed Dall to anterolateral modified Watson-Jones approach (OCM) and another surgeon changed Dall to direct anterior approach (DAA). In facility B, all 3 surgeons changed posterolateral (PL) approach to OCM. The subjects are 150 cases in total, including the each last 25 cases operated with the conventional approach and the each first 25 cases operated with a new approach (Dall to OCM: 25 + 25, Dall to DAA: 25 + 25, PL to OCM: 25 +25 cases). And, differences in operative time, intraoperative bleeding volume, postoperative hospital stay, and postoperative hemoglobin, white blood cell count, lymphocyte count, creatine kinase (CK), C-reactive protein (CRP) were investigated. Results. The average age of subjects was 64 years (31–87 years old), and there were 27 male subjects and 123 female subjects. In the change from Dall to OCM, only the postoperative hospital stay decreased significantly. In the change from Dall to DAA, the length of hospital stay and postoperative CRP significantly decreased, but the intraoperative bleeding volume increased. In the change from PL to OCM, the operation time, postoperative CRP and CK decreased, but postoperative Hb decreased. Cases with lymphocytes less than 1000/µL or less than 10% after surgery on day 4 are latent infection cases, and in such cases the operation time was significantly longer, the postoperative Hb was significantly lower, and the postoperative CK was significantly higher. However, such cases were not significantly increased by the change of operation approach. Conclusion. Introduction of the muscle sparing approach improved many items on surgical invasion, but some items deteriorated especially at the beginning of a new approach. In the early stages of introduction of the new approach, choosing cases without obesity and without lots of muscle volume may reduce latent infection

A tourniquet is usually used during ankle arthroscopy to allow for improved visibility and reduced operation time. However, clinical studies on knee arthroscopy have not demonstrated this to be true. In addition, Zengerink and van Dijk emphasized a limited tourniquet time in ankle arthroscopy as a possible factor to lower the complication rate even more. The purpose of this prospective randomized controlled trial was to examine the effect of tourniquet use on arthroscopic visualization, operative time, postoperative intra-articular bleeding, postoperative pain scores and outcome of anterior ankle arthroscopy. A consecutive series of 50 patients who were scheduled for anterior ankle arthroscopy were randomized to have the surgery done either without the tourniquet inflated (25 patients) or with the tourniquet inflated (25 patients). The patients were evaluated by the course of the surgery, postoperative intra-articular bleeding, pain during the early postoperative period and by using the subjective and objective functional scores to evaluate the condition of the ankle before and 3 and 6 months after the surgery. The statistical analysis was performed with the normality of distribution tested by both Kolmogorov-Smirnov and Shapiro-Wilk tests. Appropriate parametric or non-parametric methods were then used to test statistical hypotheses, while the statistical significance (alpha, Type I error) was set at .05. Fourty-nine patients were present at the final follow-up, 6 months after the surgery. The results between the groups were comparable regarding the duration of the operative procedure, consumption of sterile saline, visualisation and functional scores. Notable difference between the groups in favour of the non-tourniquet group was present regarding postoperative bleeding, but was not statistically significant. Statistically significant difference in favour of the non-tourniquet group was found regarding postoperative pain during several days in the early postoperative period. Our study has shown that anterior ankle arthroscopy may be performed adequately without the use of a tourniquet and that it has the same operative course as in cases in which the tourniquet is used and functional outcomes which are not worse than in cases in which the tourniquet is used

Background. Acromegaly, which stems from high level of serum growth hormone secreted by a benign tumour in the anterior pituitary gland, is likely to cause severe peripheral joint pains due to hypertrophic changes in such joints. Recently, the life expectancy of such patients has been improved and more patients with acromegaly have undergone joint surgeries to mitigate joint pain and malfunctions. However, little is known about to what extent surgical procedures can improve the joint functions of acromegalic patients compared to non-acromegalic cases. Methods. First, we qualitatively analysed prognosis of total hip arthroplasty (THA) of acromegalic patients by investigating 11 cases in which direct anterior approach (DAA) THAs were performed to 8 acromegalic patients in our hospital between 2012 and 2015. Second, we quantitatively compared the functional prognosis of the 11 cases with that of 107 non-acromegalic cases. Technically, to control the difference in age, sex, height, and weight between the two patient groups, we first identified a model that could predict 3month-/6month-/12month-functional prognosis in the control cases. We estimated differences in functional outcomes between the two groups by calculating how accurately the control-case-based model could predict the prognosis of the acromegalic cases. Results. In the qualitative analysis, we found that compared to the control, the most acromegalic cases had atypically advanced degenerative arthritides with osteophytes and enthesophytes proliferations. In addition, some cases showed other signs, such as flattering of femoral head and arthritis with slight osteophytes. Regarding surgical procedures, acromegalic cases were likely to require longer operation time and larger amounts of blood loss compared to the control. In the quantitative analysis, we first identified a model in which age and body height could predict the functional prognosis of DAA THA in the non-acromegalic cases (F[2,104] = 6.7, P = 0.0017). We then found that the actual functional outcomes of the acromegalic cases were not significantly different from those predicted by this control-case-based model (P = 0.18). Conclusions. The qualitative analysis shows the atypical joint structures and resultant prolonged operation time and blood loss in the acromegalic cases. However, the quantitative analysis could not find significant differences in prognosis between the acromegalic and non-acromegalic cases. Although these observations and analyses need to be examined in studies with large sample sizes, this work suggests that functional outcomes of DAA THA to acromegalic patients can be comparable to that to non-acromegalic patients

Robotic assisted spine surgery was a breakthrough in the evolution of spinal surgery, gradually gaining its place as an alternative technique for conventional spinal procedures. As the general population's life expectancy increased so does the incidence of spinal pathology and with it emerged an urging need for a safer and more accurate means of treatment. In our institute we apply the “Spine Assist” platform for a variety of spinal procedures as Vertebroplasties, biopsies, Pedicular screws insertion and an inter-vertebral fusion – GOLIF procedures. This study is designed to analyze the learning curve of each procedure, regarding the amount of fluoro images (FI) taken, fluoro exposure (FE) time and net operation time. All spinal procedures using the “Spine Assist” platform were included in this study; all took place from 2006 until September 2010. Exclusion criteria were procedures with failed pre-op registration, and robotic assisted procedures that were converted to conventional fluoroscopic assisted during the operation. Every single surgery of all types of procedures was analyzed regarding the amount of FI taken, FE time and net operation time. Pedicular screws insertion was grouped into sets of four, where the same parameters were evaluated. Altogether we preformed 106 robotic assisted Vertebroplasty procedures. During this period a distinct learning curve was observed and analyzed. For the first ten Vertebroplasties an average of 12 FI were taken with a net operation time of 53.6 min per procedure. Analyzing the first 40 procedures has shown less FI per procedure (5 FI) and a net operation time of 48.6 min/procedure. Data drawn from the 51 following Vertebroplasties has set the standards of 4 FI with a net operation time of 25.6 min/procedure. Two Vertebroplasty procedures were not completed due to failure of software registration. Pedicular screws are a mean for stabilization of vertebral motion units. During a six years period 706 screws were inserted, out of whom 98 were inserted using percutaneous technique. Comparing the insertion of a set of 4 screws we found a significant improvement regarding the number of FI, FE time and the net operation time between the first ten procedures and the rest with a mean of 20 FI /4 FI and net screw insertion time of 82 min/ 25 min respectively. We found no difference in the parameters comparing percutaneous Vs open Pedicular screws insertion. The mean accuracy of all procedures was 0.3 mm compared to the pre planned screw trajectory. No false route was detected in any of the 506 procedures. This robotic assisted technique is a new and safe approach aiming to shorten the duration of the procedure, thus reducing the patient and surgeon exposure to radiogenic dose. The essence of robotic assisted surgery is a pre planned needle/screw trajectory aiming to reduce the possible intra-operative complication, inaccuracies and possible mishaps emerging during “free hand” procedures. Gaining more experience using the spine assist platform, as shown in this detailed learning curve, enabled us to leverage the platform for ultra-accurate procedures as the percutaneous intervertebral fusion – GOLIF, Vertebroplasty for burst fractures etc

Outsourcing elective surgery has become increasingly commonplace to meet increasing demand from a growing & aging population. There is concern that outsourcing was influencing the nature of residual workload that was unsuitable for treatment elsewhere. This led to the impression that our unit is operating on more complex patients orthopaedic problems, ASA and Body Mass Index (BMI). By losing a disproportionate number of straightforward patients our department's outcomes, productivity and training opportunities could be adversely affected. Retrospective analysis of prospectively collected data of primary hip / knee arthroplasties between July & December for 2014(pre-outsourcing), 2015 and 2016(post-outsourcing). ANOVA, Tukey Honest Significant Difference(HSD) and Pearson's correlation used. Total of 726 primary arthroplasties were performed with an almost 50 % reduction post outsourcing. Post-outsourcing, BMI and ASA were significantly worse with a ANOVA of p=0.001 and HSD p=0.003. Length of stay increased from 5.4 days in 2014 to 6.2 days in 2015 ANOVA p< 0.001 but decreased in 2016. BMI significantly affected operating time (Pearson's r =0.12, p< 0.05) and anaesthetic time (Pearson's r =0.19, p< 0.05). ASA significantly affected length of hospital stay, p< 0.01 and operation time, p=0.007 but no effect on anaesthetic time. In conclusion, we are operating on more complex patients due to current outsourcing setup. Implications for short-term were on anaesthetic and operation time, inpatient stay and training opportunity were affected, with possible long-term implications on individual surgeon and unit outcomes (complications, patient satisfaction)

Pertrochanteric femoral fractures are common and intramedullary nailing with a proximal femoral nail (PFNA®) is an accepted method for the surgical treatment. Accurate guide wire and subsequent hardware placement in the femoral neck is believed to be essential in order to avoid mechanical failure. Malpositioned implants may lead to rotational or angular malalignment or “cut out” in the femoral neck. Hip and knee arthritis might be a potential long-term consequence. The conventional technique might require multiple guidewire passes, and relies heavily on fluoroscopy. A computer-assisted surgical planning and navigation system based on 2D-fluoroscopy was developed in-house as an intraoperative guidance system for navigated guide wire placement in the femoral neck and head. To support the image acquisition process, the surgeon is supported by a so-called “zero-dose C-arm navigation” module. This tool enables a virtual radiation-free preview of the X-ray images of the femoral neck and head. The aim of this study was to compare PFNA® insertion using this system to conventional implantation technique. We hypothesised that guide wire and subsequent implant placement using our software decreases radiation exposure to the minimum of two images and reduces the number of drilling attempts. Furthermore, accuracy of implant placement in comparison to the conventional method might be improved and operation time shortened. We used 24 identical intact left femoral Sawbones® to simulate reduced pertrochanteric femoral fractures. First, we performed placement of the PFNA® into 12 Sawbones using the conventional fluoroscopic technique (group 1). Secondly, we performed placement of the PFNA® into 12 Sawbones guided by the computer-assisted surgical planning software (group 2). In each group, we first performed open and secondly minimal-invasive intramedullary nailing in six sawbones each. For minimal-invasive guide wire placement, a surgical drape imitated soft tissue coverage. Conventional and navigated technique used a C-arm fluoroscope (Siemens IsoC 3D®, Erlangen, Germany) in conventional 2D mode. Guidewire and subsequent blade placement in the femoral neck was evaluated. We documented: 1: the number of fluoroscopic images; 2: the total number of drilling attempts; 3: implant placement accuracy (3.1. Tip apex distance (TAD); 3.2. visible penetrations of the femoral neck and head; 3.3. blade-corticalis bone distance in the anteroposterior and lateral plane) and the 4: operation time. The number of fluoroscopic single shots taken to achieve an acceptable PFNA®-blade position was reduced significantly with computer-assistance by 71.5% (p<0.001) in the open and by 72,4% (p<0.001) in the minimally invasive technique. In each operation two X-rays for final documentation were taken. The average number of drilling attempts for the computer-guided system was significantly (p<0.05) less than that of the conventional technique in the minimally invasive procedure. The average number of drilling attempts showed no difference between the computer-assisted and conventional techniques in the open procedure. Accuracy of implant placement showed no difference between the computer-assisted and the conventional group. Computer assistance significantly increased the mean operation time for fixation of pertrochanteric femoral fractures with a PFNA® by 79.8% (p<0.001) in the open technique and by 54.4% (p<0.001) in the minimally invasive technique. Use of our computer-guided system for fixation of pertrochanteric femoral fractures by a PFNA® decreases the number of fluoroscopic single shots and of suboptimal guide wire passes while maintaining blade placement accuracy that is equivalent to the conventional technique. Computer-assisted surgery with our system increases the operation time and has just been tested in non-fractured sawbones. Although these results are promising, additional studies including fractured sawbones and cadaver models with extension of the navigation process to all steps of PFNA® introduction and with the goal of reducing the operation time are indispensable before integration of this navigation system into the clinical workflow

Objectives. There are few reports on total hip arthroplasty (THA) for hip osteoarthritis associated with so-called Perthes-like change including high great trochanter, short neck hip or flattened femoral head (hereinafter called “Perthes-like change”) as the operative procedures are difficult. We studied THA for “Perthes-like change” carried out in our department. Methods. We covered 14 cases (15 hips), which underwent THA for “Perthes-like change” (hereinafter called “Perthes-like change group,” operated from 2008 to September 2011. The average age at the operation was 62 (53 to 83 years old), 7 males and 7 females, and the average follow-up period was 21.8 months (6 to 48 months). For these cases we studied the clinical items and further made a comparative review of the 258 hips as a control group (Group C), which underwent THA during the same period for osteoarthritis (OA) originating in DDH (developmental dysplasia of the hip) (Crowe type 1 and 2), excluding the “Perthes-like change group.” The items reviewed include the age at the operation, operation time and intraoperative blood loss. Results. The average JOA score of the “Perthes-like change group” at the time of the study was 89 and favorable. The average operation time of the “Perthes-like change group” was 113 minutes (69 to 202 minutes) and its average intraoperative blood loss was 1066 g (490 to 3314 g). The operation time of the “Perthes-like change group” was significantly longer compared to that of the Group C (p=0.004), and its intraoperative blood loss was also significantly larger than that of the Group C (p=0.018). We carried out the muscle release operation for 8 hips (53.3%) of the “Perthes-like change group” and we combined the retachment of the great trochanter for the 1 case of them. There was no dislocation, infection, neuroparalysis and pulmonary embolism. Consideration. The “Perthes-like change group” had a longer operation time and a larger intraoperative blood loss than those of the Group C. Also there were quite a few cases that needed muscle release operation. Therefore, care should be taken in THA for hip osteoarthritis associated with “Perthes-like change” although the clinical results were favorable

Introduction. The KneeAlign2 (OrthAlign, Inc., Aliso Viejo, CA) is a portable accelerometer-based navigation device for use in performing the distal femoral resection in total knee arthroplasty (TKA). This device works as a computer-assisted surgical system. It does not require the use of a large console for registration and alignment feedback.(image1,2). Purpose. The aim of this study was to investigate the accuracy in positioning the femoral component and the presense of a learning curve in conducting TKA using this device. Materials and methods. From May 2015 to March 2016, 60 knees underwent a primary TKA using a portable accelerometer-based navigation device for performing the distal femoral resection. These TKAs were devided in two groups. Group1: operated by surgeon of experience using the KneeAlign2 more than 30 cases. Group2: operated by surgeons of experience using the KneeAlign2 less than 30 cases. Standing AP hip-to-ankle radiographs were obtained postoperatively. Positioning of the femoral component was measured by the radiographs. Outlier in coronal alignment were defined as >3°. The radiographic results and operation time were compared between the groups. Students t-test was performed to assess the statistical analysis (p<0.05). Result. There was no outlier and all patients had an alignment within 90±3°to the femoral mechanical axis in the coronal plane in both groups. The mean deviation(absolute values) from the neutral alignment of the femoral component were 1.5±0.5 in group1 and1.2±0.7 in group2. There was no statistical significance between the groups. Average operation time was 106.2 minutes in group1 and 108.5 minutes in group2. There was no statistical significance between the groups. There were no complications during the surgery associated with the navigation device. Conclusion. This portable navigation device is highly accurate in positioning the femoral component in TKA. And as the learning curve for using this device does not be observed, this portable navigation is easy to handle even for beginner users. Purpose. Most of revision TKA needs bone reconstruction. The success of revision TKA depends on how well the bone reconstruction can be done. The method of reconstruction includes bone cementing, metal augmentation, allogenic bone graft, APC and tumor prosthesis, etc. In moderate to severe bone defect, allograft is needed. However, allogenic bone graft is surgically demanding and needs long operation time, which is very risky to the elderly patients. The authors revised an alternative method of bone defect reconstruction using cementing method with multiple screws augmentation. Methods. There were 12 cases of patients with large defect which could not be reconstructed with metal augment from April 2012 to April 2014. The authors performed 3 to 5 screws fixation on the defect site. Sclerotic bone is prepared with burring for better cementing. 3 ∼ 5 screws according to the size of defect. The length of screw fixation was determined as deep to the bone until stable fixation just beneath the implant. When drilling for the screw insertion, intramedullary guide is put into the medullary canal so as not to interfere with implant insertion. The defect is filled with cement during prosthesis fixation. Weight bearing was permitted on postoperative 3rd day, as usual manner of primary TKA. Results. According to the AORI classification, there were 10 cases of 2A and 2 cases of 2B. Mean follow up period was average 15 months. The number of screw insertion was 4.3 ea (2∼8). Average operation time was 1 hour and 57 minutes. Mean ROM was 107.9. HSS score, KSS score ad WOMAC score were 86.3, 92.8 and 11, respectively. There were no case of infection and loosening at the last follow-up. Conclusion. Cementing with multiple screws augmentation technique is a good alternative of bone reconstruction

Aim. Reveal the rate of surgical site infection (SSI) after primary hip and knee arthroplasty in patients with inflammatory joint disease and analyse if the infection rate was correlated to the given anti-rheumatic treatment. The background is that since 2006 patients operated at the orthopaedic department at Skåne University hospital, Lund, Sweden, have continued treatment with TNF-alpha inhibitors during the perioperative period. Method. During 2006 to 2015 494 planned primary hip and knee arthroplasties were performed on 395 patients (236 hip arthroplasties and 239 knee arthroplasties). Data on age, sex, diagnosis, BMI, operation time, ASA-classification, treatment with cDMARDs (conventional disease modifying anti-rheumatic drugs) and bDMARDs (biological disease modifying antirheumatic drugs) and use of prednisolone was collected. The primary outcome variable was prosthetic joint infection (PJI) within 1 year from surgery with a secondary outcome variable being superficial SSI. Results. In 32% (n=159) of the cases the patient was treated with a TNF-alpha inhibitor. The rate of PJI was 1.4% (n=7). The overall rate of infection, including superficial infections, was 2.4% (n=12). All the PJIs occurred after a knee arthroplasty and only 1 patient was treated with a TNF-alpha inhibitor (etanercept). Conclusions. We could not find that continuing treatment with TNF-inhibitors perioperatively led to a higher incidence of PJI or SSI than generally would be expected in a group of patients with an inflammatory joint disease. Based on these results there is no need to discontinue treatment with TNF-inhibitors when performing arthroplasty surgery

Femoral trans-trochanteric rotational osteotomy(TRO) is widely used for the younger patients who have idiopathic osteonecrosis of femoral head. However, it usually converts to THA, and in hip arthroplasty, the procedure is commonly technically demanding and surgeon needs higher level of surgical technique. This study was designed to investigate comparison between simple THA and THA following TRO, and indicates specific surgical techniques of THA following TRO. Materials and Methods. Between 2002 and 2012, the senior authors performed 20 cementless THA in 16 patients who had followed after TRO. The operation time, blood loss, post-operative X-ray findings, and overall clinical results (JOA score) were investigated. These data were compared with simple THA following 10 years. Results. A mean duration between TRO and THA was 58 month, a mean age at the time of THA was 52.1 years old. Pre-operative classification was all Type C and stage 2 or higher stage. Osteonecrosis of femoral head before THA was mean 56.2%. Operation time was 160 minutes, and mean blood loss was 764 g. In post-operative X-ray findings, a cup was at original anatomy in place in all cases. There is a tendency of slight varus femoral stem position after THA following TRO. The JOA score was improved from 42.1 to 74.2. Discussion. Technically, there are a couple of issues in THA following TRO. Specific surgical techniques are needed. Surgical results depend on hip range of motion before THA. There is a difficulty of improvements in ROM after THA following TRO

Background. The Robotic Spinal Surgery System (RSSS) is a robot system designed for pedicle screw insertion containing image based navigation system, trajectory planning system and force state recognition system. The special force state recognition system can guarantee the safety during the operation. The RSSS is helpful in pedicle screw insertion surgery and it will be applied in clinic in the near future. In this study, we evaluated the accuracy and safety of RSSS in an animal experiment. Methods. Computer tomography (CT) scan data for two anesthetised experimental sheep was acquired using the C-arm and transferred to RSSS for pre-surgery screw trajectory planning. With the assist of RSSS, we inserted 8 and 4 screws into two sheep respectively. Operation time and blood loss during the surgery were recorded, and CT scan was repeated after surgery. Real screw position and trajectory acquired by the post-surgery CT scan and ideal trajectory planned by RSSS were compared to evaluate the accuracy and safety of RSSS. The result is shown as mean±SD. Results. We planted totally 12 screws into two sheep. The operation time for each sheep is 140min and 110min, and the blood loss is 100ml and 80 ml respectively. Compared with planned trajectory, the average deviation of the entry points in lateral and axial view are 1.07±0.56mm and 1.25±0.42mm and the mean screw deviation angles in later and axial view are 1.78±0.98°and 2.52±1.03°respectively. The RSSS successfully recognised the force stages and guaranteed the safety during the drilling process. There is no penetration in all 12 pedicles, and all the screws fell into group A according to the Gertzbein-Robbins classification. Conclusion. This animal study demonstrated the accuracy and safety of the RSSS, which also supported the potential application in clinic

Introduction. A large proportion of patients with osteoarthritis of the knee, present with bilateral symptoms at the outpatient department. A simultaneous total knee arthroplasty (TKA) procedure is available for such patients. The first operation in a simultaneous surgery may provide information to the operator to determine component size, soft tissue balancing, and estimate gap size for the second operation, while the second team usually conducts an operation in a confined space on the contralateral side during closure for the first operation, which can disturb cooperation during the second operation and may lead to more intra-operative surgical errors. We hypothesized that the circumstances of the two consecutive operations of a simultaneous bilateral TKA are different, could lead to different outcomes of overlapping bilateral TKAs. We therefore addressed the following research questions to determine whether there would be differences in short-term clinical outcomes, radiographic results, and implanted component size between the two sides. Methods. A retrospective review of 451 consecutive patients, who underwent simultaneous bilateral TKA between January 2011 and April 2012, was conducted. Bilateral TKAs were performed with the senior surgeon conducting the main procedure (from skin incision to implantation of first prosthesis until prior closure of the first knee) on the right side first and subsequently the left side with a second team. At 1 year after surgery, clinical outcome scores (the Knee Society Knee and Function scores, WOMAC score), radiologic findings were evaluated and clinical results as postoperative blood loss, operation time were compared between bilateral sides. Results. A greater incidence (16.1%) of outliers during postoperative coronal limb alignment (>±3o) were identified in the second TKA than those in the first TKA (9.0%) (p =0.003). Multivariate analysis for the association of outlier rates in the second TKA relative to the first revealed a significance for severity of the preoperative deformity (pre-op. coronal limb alignment, p = 0.002) and decreased ROM (p = 0.042) from the GEE analysis. The second knee also showed more blood loss (735 vs. 656mL), and longer operation time (61, 58 minutes respectively), as compared to the first TKA, while no significant differences in clinical outcomes. Discussion and Conclusion. There were no significant differences in the clinical outcomes even though few distinct outcomes due to different circumstances of the surgery. Awareness of these findings can help the continued success of bilateral TKA in an increasing patient population

Introduction. Total hip arthroplasty (THA) for a highly dislocated hip can be problematic and technically challenging. Our previous study on cemented THA with subtrochanteric femoral shortening osteotomy revealed a high incidence (20%) of non-union. Therefore, in 2008, we introduced reverse hybrid THA using S-ROM stem for the treatment of a highly dislocated hip. The purpose of this study was to assess the short-term clinical outcomes of this new method. Patients and methods. Between 2008 and 2014, 13 consecutive reverse hybrid THAs were performed on nine female patients with highly dislocated hips. The average age at the time of operation was 66 years (range, 55–85 years). The acetabular component was fixed in the true acetabulum with bone cement. Transverse osteotomy was performed below the lesser trochanter to shorten the femur and to prevent over-lengthening. The proximal sleeve of the S-ROM stem was then fixed within the proximal fragment, and the distal fin provided rotational stability of the distal fragment. Thus, the two fragments were fixed to each other with the S-ROM stem, and the resected segment was longitudinally cut for grafting at the junction. The postoperative follow-up period was an average of 4 years (range, 1–7 years), and no patients were lost. Preoperative and final Japanese Orthopaedic Association (JOA) hip score, operation time, bleeding amount, intraoperative and postoperative complications, bone healing at the osteotomy site, implant loosening, and revision surgery were retrospectively investigated. Results. The mean JOA hip score improved from 56 points preoperatively to 82 points postoperatively. The operation time and amount of bleeding were an average of 208 min and 643 g, respectively. The mean length of femoral resection was 4 cm (range, 2–6 cm), and the tip of the greater trochanter migrated an average distance of 7 cm (range, 5–9 cm) distally. The calculated limb lengthening was an average of 3 cm (range, 2–4 cm). Intraoperative fracture was seen in two patients, but no repeat operation was required. Two patients experienced postoperative dislocation in their hips, but additional surgery was not necessary. Postoperative nerve palsy did not occur in any patient, and all the osteotomy sites showed complete bone union. There was no implant loosening seen in any patient, and there was also no need for revision surgery. Discussion and conclusions. To achieve satisfactory outcomes with this method, resection of necessary and sufficient length of femur and accomplishment of adequate fixation between the proximal and distal fragments are necessary. In this study, dislocation occurred in two patients, and no nerve palsy was seen. Larger femoral heads may be recommended to eradicate dislocations. In cases where metaphysis of the femur is hypoplastic or the medullary canal is wide, reaming and stem insertion should be carefully performed. In this series, no additional surgery was required for the intra- and post-operative complications, and the osteotomy sites achieved bone union in all patients. Therefore, we recommend the use of this method, although longer follow-up periods are necessary

Introduction. Patient matched instrumentation (PMI) have been proposed the accuracy of bone cuts through custom cutting blocks and provide the proper alignment of total knee arthroplasty (TKA). On the other hand, there are some reports that the introduction of PMI for guiding bone cuts could increase the incidence of malalignment in primary TKA. Recent comparisons between patient-specific cutting guides and quantitative assessments of postoperative alignment have revealed the presence of outliers with respect to coronal alignment. The purpose of this study was to assess the implanted component alignment post-operatively between one type of MRI based PMI (Visionaire; Smith & Nephew, Inc, Memphis, Tenn) and conventional surgical instrumentation (CI) using radiographs and CT scan. Methods. 32 knees in 32 patients (25 women) with medial type knee osteoarthritis were underwent cruciate retaining TKA between September 2013 and May 2015, and were included in this study. Preoperative MRI scanning of the hip, knee, and ankle was performed for PMI group (n=12) and CT scanning was performed for CI group (n=20) 6 weeks before surgery according to a standard scanning protocol to determine the surgical epicondylar axis (SEA). Postoperatively, we compared operation time, blood loss, and mechanical alignment of two groups. Post-surgical mechanical alignments such as hip-knee-ankle angle (HKA), frontal femoral component angle (FFC), and frontal tibial component angle (FTC) were determined using long leg radiographs (Fig. 1). CT scans were used to assess the condylar twist angle (CTA) made by SEA and posterior condylar axis (Fig. 2). Each measurement was performed by two, blinded independent observers, and interclass correlation for each measurement was calculated. A student's two-tailed t test was used to compare the two cohorts, with statistical significance set at a p-value of <0.05. Results. There was no significant difference between the two groups regarding preoperative age, body mass index, degree of mechanical deformity, mean operation time or mean blood loss. Mean pre / postoperative HKA in the PMI group was 166.8 ± 6.4°/ 176.7 ±1.9° and 168.3 ±8.3° / 178.8 ±2.1°in the CI group. There was no significant difference between two groups, but there were fewer ± 3° HKA outliers with CI group (20%) than with PMI group (50%). Postoperative FFC was 89.1 ± 0.6° in PMI group and 88.1 ± 1.2° in CI group. FTC was 87.8 ± 1.4° in PMI group and 89.3 ± 1.6° in CI group. CTA was −0.8 ± 0.6 ° in PMI group and −1.2 ± 1.1° in CI group. There was no significant difference between PMI and CI group in postoperative mechanical alignment, but there were higher ± 3° FTC outliers in PMI group (50%) compared to CI group (10%)(p=0.01). (Fig.3). Discussion and Conclusion. Although the overall mean mechanical alignment for PMI group was similar to CI group, these results suggest that PMI did not significantly improve alignment.
